test(#3090): stop exempting violations under categories that do not fit
An allow-test-rule annotation citing a category that does not apply is worse than no annotation, because it reads as reviewed. Eight were confirmed by reading the assertions each one covered, and auditing the rest found five more plus one refutation — a converter test whose wording described the wrong mechanism while the covered assertion genuinely was deployed-text. The instructive one used the CANONICAL string for the same mistake: STATE.md command output labelled as a deployed artifact. A canonical string is not evidence the category fits, which is why normalising strings alone would have laundered the problem rather than fixed it. Every mapping the audit had inferred rather than code-verified was spot-checked before rewriting, and the ones that turned out not to fit were re-annotated rather than relabelled. Fourteen STATE.md assertions had a typed extractor available all along and now use it; their annotations came out because nothing needs exempting. Eight assertions genuinely need a production change first — CLI stdout and stderr with no structured mode — and are tagged pending-migration-to-typed-ir citing #3090, which is what that category is for. It had zero real uses before this, while one file carried a real citation to migration issue #2974 under a non-canonical tag. Six annotations covered assertions that do no text matching at all. An exemption for a violation that does not exist is noise that makes the real ones harder to audit; those are removed. atomic-write-coverage gains the annotation it always warranted — its own docstring describes a structural-regression-guard while the file carried none. Fifty-nine non-canonical strings across roughly thirty files are normalised, and the allow-test-rule allowlist is regenerated to match. 472 annotations became 463: every one now uses a canonical category, and the two remaining non-canonical strings are ESLint RuleTester fixtures, not annotations. fix(#2868): resume a phase stranded between its last plan and verification (#3041)
* fix(#2868): resume a phase stranded between its last plan and verification discover_and_group_plans exited unconditionally once every plan was filtered out, conflating "no plan work left" with "phase fully done". Those differ once a run can be interrupted between the final wave's SUMMARY and the verify step -- most often by a checkpoint plan that is retired but still writes a SUMMARY. The result was a phase that looked healthy from every index yet had no VERIFICATION.md, and whose recommended recovery command provably no-opped, because the only step that produces the artifact sits ten steps past that exit. The exit is now conditional. When the verification report is genuinely missing and no filter is active, the run reports the situation by name and continues at the tail gates instead of stopping. Two guards keep the normal paths untouched: - A filtered run (--gaps-only, or an explicit wave) finding nothing left in its own slice says nothing about whether the phase as a whole is done, so it exits exactly as before. Without this, --wave 1 on a finished first wave would jump to verification with later waves still outstanding. - A phase that already has its report exits as before too. The recovered path deliberately keeps the code-review and regression gates. The manual workaround this replaces skipped both, and that gap is the reason a real route exists rather than telling users to spawn the verifier by hand. Also acknowledges the emitted growth of the workflow file. As with #2830 it is appended to the fragment that already owns that path, since the linter hard-fails when two acknowledgment sources name the same one. BLOCKER -- the trigger conflated two different zero-runnable states. This step now has two skip rules: has_summary (the #2868 target) and, from #2830, a skip for plans whose blocked_by is non-empty. "All filtered" was therefore reachable with plans that never ran: plan A halts and is summarized, plan B is blocked by A and has no summary. The resume path fired, announced "All N plans are summarized" -- false -- skipped the wave steps so B was never dispatched, and jumped to the gates. B was silently abandoned, which is the same class of disappearance #2830 exists to prevent, reintroduced one layer up. The decision is now an explicit ordered three-way: a filtered run exits unchanged; any blocked-plan skip reports the phase as stuck on a halt and exits, routing to resolving the halt rather than to verification; only an all-summarized, unfiltered phase with a missing report resumes. MAJOR -- RESUME_TAIL_ONLY was set and never read anywhere in the workflow or its step fragments. Dead state implying enforcement that did not exist. Removed; the imperative at the decision point is what actually carries the control flow, so it now says so plainly. MAJOR -- the resume path skipped aggregate_results, which is the only step that runs the secure-phase threats-open gate. A phase with open threats would have advanced with no warning where a normal run always shows one. The path now enters at aggregate_results, verified to read only on-disk phase artifacts and independent queries, so it tolerates having executed no plans this run. * chore(#2932): emit a per-invocation section manifest from init Extends the init bundle with a typed per-invocation section manifest so an invocation loads only the branch guidance it will actually take. The three flag/state-gated branches in execute-phase.md move into their own step files; the parent keeps its gsd:section markers wrapping a one-line on-demand reference, so each section's prose lives in exactly one file and the parent shrinks 93369 -> 89507 bytes. A new drift-guarded generator derives the shipped section manifest from those markers, and a new pure evaluator maps invocation facts to applicable section ids. The evaluator is a lookup over the frozen WHEN_VOCABULARY, never a parser (Greenspun's Tenth Rule, ADR-1671:69); a parity test asserts the vocabulary and the predicate map stay exhaustively in sync. Closes #2932 * fix(#2932): fail closed on prototype-chain when values An isolated adversarial review found WHEN_PREDICATES[section.when] was a bracket lookup on a plain-prototype object, so inherited Object.prototype members resolved as predicates: "constructor"/"toString"/"valueOf"/ "hasOwnProperty" returned truthy and SILENTLY INCLUDED the section, and "__proto__" threw an untyped TypeError carrying no .reason. Both violate the module's documented fail-closed contract, and the manifest is read from disk at run time so it cannot be assumed trustworthy. Builds the predicate map on a null prototype and guards the lookup with an explicit Object.hasOwn check. test(#1973): consolidate 48 workflow regression tests into workflow-aspect suites
Fold 48 issue-named workflow-markdown regression files into the canonical test that owns each workflow aspect (execute-phase-*, plan-phase-*, quick-*, discuss-*, worktree-cleanup, secure-phase, verify, update, settings, etc.), across 32 existing suites. Verbatim block-scoped describe wrappers; 281 subtests conserved 1:1. No new test files. Host-env pre-check: only gsd-settings-advanced spawns CLI and it sets no GSD_WORKSTREAM/GSD_PROJECT value — no leak risk. Regenerates regression-name allowlist (222->181), ratchets file-count allowlist (verify 11->10), makes 30 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456; prunes 30 stale ids). lint:ci green. * chore(#604): rename get-shit-done/ runtime directory to gsd-core/ Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary (`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers are unaffected. Mechanical (bulk, ~90% of the diff): - `git mv get-shit-done gsd-core` - Swept path/identifier references across the repo via `perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead preserves the five legitimate slug variants that are NOT the directory: get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names). - Build/manifest wiring: package.json (bin, files, coverage globs), tsconfig.build.json (outDir), ~86 .gitignore build-output entries, stryker.config.mjs, scan-ignore files, install.js path strings. - Frozen (not rewritten): CHANGELOG.md history; translated docs (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/). New logic (review here): - src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper ADR-0008 installer migration. On upgrade it walks the legacy `~/.claude/get-shit-done/` tree, classifies each file via the prior install manifest, and emits remove-managed / backup-and-remove for managed files while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked root and symlinked entries; bounds-checks every path under configDir). The framework rolls back on install failure. Emptied dirs may remain (framework has no recursive dir-removal primitive) — documented. - scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare `get-shit-done` directory token (split token to avoid self-match; case- insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines). Wired into the lint-tests CI job. - Restored scripts/lint-package-identity-drift.cjs detection regexes (the mechanical sweep had wrongly rewritten the old-name patterns it exists to detect) and marked them as intentional legacy references. - TDD tests for the migration and the guard; do.md slash-command guard regex tightened so a `/gsd-core/bin` path segment is not mistaken for a command; changeset + docs/installer-migrations.md row added. Breaking: the installed runtime path moves `~/.claude/get-shit-done/` -> `~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed files (preserving user files) on upgrade. * fix(3266): preserve wave 0 and bucket plans by depends_on DAG in phase-plan-index Fixes two cooperating bugs in the phase-plan-index builder: 1. Wave 0 collapse: `parseInt(...) || 1` coerced parsed value `0` to `1` due to JS falsy default. Fixed with `Number.isNaN` guard. 2. depends_on ignored: wave-bucketing used only the `wave:` frontmatter field. Now replaced with Kahn's topological-level algorithm over `depends_on`: source nodes (no in-phase deps) → lowest level; each plan's level = max(deps' levels) + 1. Declared `wave:` that disagrees with computed level emits a non-fatal warning on the result. Cycle detection throws GSDError. `PlanInfo` gains `depends_on: string[]`. `PhasePlanIndex` gains `warnings?: string[]`. Both TS (`sdk/src/query/phase.ts`) and CJS twin (`get-shit-done/bin/lib/phase.cjs`) fixed identically. - code-review: config-set for code_review / code_review_depth replaces 2 greps; removes CONFIG_PATH constant
- thinking-partner: config-set features.thinking_partner replaces two greps (config-schema.cjs AND core.cjs)

fix(#2543): replace legacy /gsd-<cmd> syntax with /gsd:<cmd> across all source files (#2595)
Commands are now installed as commands/gsd/<name>.md and invoked as /gsd:<name> in Claude Code. The old hyphen form /gsd-<name> was still hardcoded in hundreds of places across workflows, references, templates, lib modules, and command files — causing "Unknown command" errors whenever GSD suggested a command to the user. Replace all /gsd-<cmd> occurrences where <cmd> is a known command name (derived at runtime from commands/gsd/*.md) using a targeted Node.js script. Agent names, tool names (gsd-sdk, gsd-tools), directory names, and path fragments are not touched. Adds regression test tests/bug-2543-gsd-slash-namespace.test.cjs that enforces zero legacy occurrences going forward. Removes inverted tests/stale-colon-refs.test.cjs (bug #1748) which enforced the now-obsolete hyphen form; the new bug-2543 test supersedes it. - Replace require('node:assert') with require('node:assert/strict') across
all 73 test files to enforce strict equality (no type coercion)
- Replace try/finally cleanup blocks with t.after() hooks in core.test.cjs
and hooks-opt-in.test.cjs per the test lifecycle standards
- Utility functions in codex-config and security-scan retain try/finally
as that is appropriate for per-function resource guards, not lifecycle hooks

* perf(tests): add --test-concurrency=4 to test runner for parallel file execution
Node.js --test-concurrency controls how many test files run as parallel child
processes. Set to 4 by default, configurable via TEST_CONCURRENCY env var.
Fixes tests at a known level rather than inheriting os.availableParallelism()
which varies across CI environments.

XML (meaning "a human should verify this step"), which matches the
scanner's fake-message-boundary pattern for LLM APIs. This is a
false positive — add it to the allowlist alongside the other test files
that legitimately contain injection-adjacent patterns.

Adds `workflow.use_worktrees` config option (default: `true`) that allows users to disable git worktree isolation for executor agents. When set to `false`: - Executor agents run without `isolation="worktree"` - Plans execute sequentially on the main working tree - No worktree merge ordering issues or orphaned worktrees - Normal git hooks run (no --no-verify needed) This provides an escape hatch for solo developers and users who experience worktree merge conflicts, as worktree ordering issues are inherently difficult when parallel agents modify overlapping files.
