1fe85cd43e chore(#4244): ESLint rules for the #4220 Windows dirname-walk / TMPDIR-triad bug class (#4246)
* fix(#4244): repoint TEMP/TMP alongside TMPDIR and fix the sweepProtectSet fixed-point walk

Repo-wide sweep (ahead of adding lint rules for these exact bug classes)
found both incident patterns still live and unfixed on `next`:

- scripts/run-tests.cjs's sweepProtectSet walk stopped on
  `cur !== runTempRoot && cur.length > 1` — a POSIX-only sentinel.
win32 dirname('D:\') is a fixed point (length 3, never satisfies
`> 1`... wait, it does satisfy length>1), so a selected file living
outside runTempRoot (the common case) spins the walk forever on
Windows. Extracted a pure, exported computeSweepProtectSet helper
that terminates on dirname(cur) === cur instead, with in-process
RuleTester-style coverage for both win32 and posix paths.

- tests/run-tests-temp-root.test.cjs's own #4020 regression test set
only TMPDIR on its runNode(...) child env. Node's os.tmpdir() never
reads TMPDIR on Windows (only TEMP, then TMP), so the redirect
silently no-oped there — masked because Windows CI died in the
dirname-walk hang above before ever reaching this test.

- tests/config-schema.property.test.cjs's fallow config-set test had
the same TMPDIR-only pattern, direct process.env assignment this
time, restored in its own finally block.

Origin: #4220 and its shared root cause #4020.

* feat(#4244): require-full-tmpdir-triad and no-unbounded-dirname-walk ESLint rules

Two custom local ESLint rules catch the #4220 / #4020 Windows CI hang bug
class at author time, joining the ADR-1703 DEFECT.WINDOWS-TEST-PORTABILITY
catalog. Neither eslint-plugin-unicorn nor eslint-plugin-n has a rule for
either shape.

- local/require-full-tmpdir-triad: flags a TMPDIR environment override
(direct process.env.TMPDIR assignment, or a TMPDIR property in a
spawn-like call's env: object literal) not accompanied by TEMP and TMP
in the same scope. Node's os.tmpdir() never reads TMPDIR on Windows.
Registered on tests/**/*.cjs, matching the require-userprofile-with-home
  precedent.

- local/no-unbounded-dirname-walk: flags a while/do-while loop reassigning
from dirname() with no fixed-point termination guard
  (dirname(cur) !== cur, or path.parse(cur).root). path.dirname() is a
no-op at the platform root, but the value differs by platform
  (win32 'D:\' is length 3, posix '/' is length 1), so a POSIX-shaped
length/equality bound never fires on Windows. Registered on BOTH
tests/**/*.cjs and scripts/**/*.cjs — the real #4020 bug lived in
  scripts/run-tests.cjs, not tests/.

Both rules join the zero-escape-hatch discipline already established for
this catalog (no bespoke comment marker; PROTECTED_RULES in
tests/portability-rule-disable-ban.test.cjs independently bans
eslint-disable of either). ADR-1703 and its two companion contributing
docs get an amendment documenting the mechanism, code examples, and the
repo-wide sweep (three live instances found and fixed in the prior
commit; no others found). CI test-scope selection updated so an edit to
either rule or to scripts/run-tests.cjs re-runs the right suites.

* fix(#4244): no-unbounded-dirname-walk must analyze a single-condition loop test too

checkWhile bailed out early unless node.test was a LogicalExpression,
so a single-condition loop -- while (cur !== root) { cur = dirname(cur); } --
was silently skipped and never reported. That is the EXACT minimal
shape of the original #4020/#4220 bug, and it is literally the shape
used by this rule's own shipped RuleTester fixtures (the "equality-only
bound" invalid cases), which were failing (0 errors reported, 1
expected) until this fix -- confirmed by running RuleTester directly
against both fixtures, not just via a passing test-runner exit code.

The conjunct-collection helper already handled a non-LogicalExpression
test correctly (it pushes a single node as the sole conjunct); only the
early-return gate needed to stop requiring a compound && / || test.

Verified: RuleTester run directly against both previously-broken
fixtures plus two new sanity cases (a guarded single-condition loop
stays valid; an unrelated single-condition loop stays silent), and a
fresh `npx eslint .` across the whole repo remains clean (no other
single-condition dirname-walk shape exists in the tree).

* fix(#4244): require-full-tmpdir-triad must recognize a destructured child_process call

isSpawnLikeCallee only recognized a MemberExpression callee
(child_process.spawnSync(...)) or a bare identifier in
ENV_LOCAL_HELPER_NAMES (runNode). A destructured import called bare --
const { spawnSync } = require('child_process'); spawnSync(...) -- has an
Identifier callee named "spawnSync", which matched neither branch, so
the whole env-literal check was skipped. gsd-test caught this: both
"invalid: child_process.spawnSync with TMPDIR-only env" cases in
tests/require-full-tmpdir-triad.rule.test.cjs were failing (0 errors
reported, 1 expected).

Widened the bare-identifier branch to also match any of the known
ENV_CHILD_PROCESS_METHODS names, matched by name only -- the same
lightweight convention this repo's other eslint-rules/*.cjs use (e.g.
no-hardcoded-tmp.cjs's isFsMethodCall), not full import data-flow
tracing.

Verified: RuleTester run directly against all 11 cases in
tests/require-full-tmpdir-triad.rule.test.cjs (not just the two that
were failing), all pass; a fresh npx eslint . and npm run lint:ci
across the whole repo remain clean.

* fix(#4244): correct a stale escape-hatch reference in a test comment

The comment on the "length comparison against another expression's
length" case referenced a "// allow-dirname-walk marker" that doesn't
exist -- the rule has zero comment-based escape hatches by design
(ADR-1703), and an earlier draft's marker mechanism was removed before
this branch's first commit. Spec-axis review caught the stale
reference. No behavior change; comment-only.

69e7afd0c7 chore(#3212): bounded quantifiers over document content — prohibition with teeth — Phase 4 (#3441)
* feat(#3415): ship local/no-unbounded-quantifier, burn down ReDoS class

Phase 4 of epic #3212 (ADR-3212 §5/§7, the final phase). New rule flags
an unbounded */+/{n,} quantifier over a broad character class
([\s\S], dotAll ., or a 1-2-unit negated class like [^\n]/[^)\n] — the
exact #2128-fixed shape) applied to a regex whose match target is
data-flow-traced to readFileSync content.

eslint-rules/lib/readfilesync-trace.cjs extracts the data-flow tracer
shared with no-crlf-fragile-split (Phase 2) rather than a second copy
— no-crlf-fragile-split refactored onto it with zero behavior change,
parity-tested.

Real triage, not 798 mechanical edits: the ADR's census (2026-08-08)
screened every unbounded quantifier in the tree unscoped. Correctly
scoped to readFileSync-derived content (matching Phase 2's own G2/G3
scoping), the rule found 162 real hits across two detection waves — the
second wave (93) surfaced only after a genuine off-by-one bug in this
rule's own first draft was caught while writing its RuleTester tests
and fixed (the bug silently missed every directly-quantified [\s\S]*
with no gap before the quantifier — exactly the class this rule exists
to catch). 3 hits landed in production src/ (commands.cts, milestone.cts,
roadmap.cts) and were each empirically timed against adversarial input
(matching #2128's own measured-not-assumed precedent) — all confirmed
linear-time/benign, left unbounded with a measured-evidence comment
rather than mechanically bounded. The remaining 159 are test-file
fixture parsing (test-author-controlled, fixed-size content, not
adversarial input) — each suppressed with a specific, non-generic
reason. Zero functional behavior changed anywhere in this diff.

tests/no-pending-3212-markers.test.cjs locks the epic's own closing
invariant (ADR §7: "assert zero pending #3212 markers remain") — ground
truth confirmed trivially true today (no phase left any such marker
behind), now regression-locked going forward.

* fix(#3415): correct rule category mislabel, add CI test-scope entry

An orthogonal Standards-axis review found eslint-rules/no-unbounded-quantifier.cjs
mistakenly carried meta.docs.category: 'Portability', copied from a sibling
rule without realizing what that implied: docs/contributing/cross-platform-
portability-rules.md governs an ADR-1703 rule family under a hard "zero
escape hatches" contract (tests/portability-rule-disable-ban.test.cjs's
PROTECTED_RULES bans eslint-disable for those rules entirely). This rule is
not part of that family — it's ADR-3212 (ReDoS/CWE-1333), a different epic —
and its eslint-disable-next-line suppressions (159 of them, added earlier
this same phase after empirical benign-verification) are an intentional,
correct design, not a bypass. Corrected to category: 'Best Practices',
matching the actual precedent (no-adhoc-regex-escape.cjs, Phase 1 of the
same epic, which is also correctly outside PROTECTED_RULES), and the rule's
own docstring now states this explicitly so a future reader doesn't have to
re-derive it.

Also registers a new scripts/ci-test-scope.cjs bucket so editing this rule
or the shared eslint-rules/lib/readfilesync-trace.cjs helper re-runs their
own test suites under targeted CI selection — was previously unregistered
and invisible to that fast-path (this PR's own gsd-test checkpoint runs the
full suite regardless, so this only affects future narrowly-scoped PRs).

* fix(#3415): bound no-unbounded-quantifier's own scanner (CWE-1333, ironic)

Security review found the rule meant to catch algorithmic-complexity bugs
had one of its own: hasUnboundedBroadQuantifier's negated-class inner
scan walked from each `[^` occurrence to the next `]` (or EOF) with no
bound, while the outer loop only ever advanced by one character — O(n²)
total work on a pattern with many unclosed `[^` runs. Runs unconditionally
inside checkPattern on any `new RegExp('literal string')` argument in any
linted file, before the (cheap) readFileSync data-flow gate — so a single
crafted string literal, no valid regex syntax required, could make
`npm run lint` / CI hang.

Empirically confirmed both the bug and the fix: pre-fix, n=4000/8000/
16000/32000 chars took 30.8/115.6/463.8/1874.3ms (~4x work per 2x n,
quadratic); extrapolated, the 300000-char repro from the finding would
run ~165s. Post-fix (bail the inner scan once units exceeds the rule's
own 1-2-unit scope, rather than continuing to hunt for a closing `]`),
the same 300000-char input runs in 8.7ms via the real rule module,
independently reconfirmed at 18ms via a fresh Linter.verify() call.

New regression row in tests/no-unbounded-quantifier.rule.test.cjs
asserts the RuleTester run on a 50000-char adversarial pattern
completes and returns a defined result — no wall-clock assertion
(CLAUDE.md Clock Seams / local/no-elapsed-assertion).

* fix(#3415): triage 3 new sites, re-raise ceiling after upstream batch

next merged 12 more PRs during this PR's review. Two consequences:

- tests/edit-phase.test.cjs (fix #3262, unrelated) added 3 new
content.match(/<tag>([\s\S]*?)<\/tag>/) reads of this repo's own
workflow .md content — the same Class A pattern as the ~159 sites
already triaged elsewhere in this PR. Suppressed with the same
  established reason.
- lint-allow-test-rule-refs' ratchet ceiling needed re-raising again
(301 -> 303) for the same reason as the two prior bumps: organic
growth from unrelated, already-reviewed PRs landing concurrently,
not a defect in this branch's own diff.

dc3c81e93d chore(#3212): src/pattern.cts is the sole owner of runtime-value regex construction — Phase 1 (#3416)
* test(#3412): failing-first suite for the pattern-construction seam

Phase 1 of epic #3212 (ADR-3212 §1/§2/§7). Tests only — src/pattern.cts
and eslint-rules/no-adhoc-regex-escape.cjs do not exist yet, so both
suites fail with MODULE_NOT_FOUND, which is the intended RED.

Locks the measured behavior rather than the assumed behavior:
RegExp.escape hex-escapes the leading character of nearly every string
("abc" -> "\x61bc"), so the suite asserts match-equivalence against an
inlined historical oracle (the implementation being deleted) rather
than byte-equivalence of pattern text — 200 seeded fast-check runs plus
a fixed corpus, 0 mismatches. Also locks the latent character-class
range bug this phase fixes as a side effect: a hyphen-bearing value
interpolated into [...] currently forms a real range and matches an
unintended character; post-migration it must not.

* chore(#3412): src/pattern.cts owns runtime-value regex construction

Phase 1 of epic #3212 (ADR-3212 §1/§2/§6/§7). Adds the pattern seam
delegating to the built-in RegExp.escape, deletes every hand-rolled
copy, and raises the Node floor to the Active LTS line.

The census was low, three times over. ADR-3212 counted 10 copies; a
graph query found 12; the new lint rule — once live — found 27 more.
The difference is that the census counted named helper FUNCTIONS while
the rule counts the escape SHAPE, so inline .replace(<class>, '\$&')
copies were never in scope. ADR §1's actual requirement is that no
module outside the seam escapes a value for regex use, so all of them
are, and CLAUDE.md's no-defer rule makes them this change's work.
Fourth consecutive epic here whose copy count was low — the argument
for ADR-3180 Amendment 3's "state N found by the guard" rule.

Also corrected mid-implementation: the survey reported phase-id.cts's
escapeRegex had 0 external importers. It had 8 production importers,
making its removal a public-surface change to an ADR-2121-owned module
and requiring an update to that ADR's locked-surface test. Blast
radius revised Medium-High -> High.

RegExp.escape is match-equivalent but NOT text-equivalent: it
hex-escapes the leading char of nearly every string ("abc" ->
"\x61bc"). Equivalence is proven by a seeded fast-check property test
against the deleted implementation as oracle. It also fixes a latent
bug: a hyphen-bearing value interpolated into a character class
previously formed a real range and matched an unintended character.

Node floor 22 -> 24 (RegExp.escape is Node 24+), across engines,
.nvmrc, package-lock, 9 CI matrix entries, and 5 docs. The aggregate
`required-tests` context is unchanged and no job was added or removed,
so branch protection cannot be orphaned by the dropped lanes.

Enforced by eslint-rules/no-adhoc-regex-escape.cjs (shape-matched, with
structural provenance for reviewed pattern-fragment constants rather
than a name heuristic) plus a whole-tree companion guard covering the
directories ESLint's globs miss.

* fix(#3412): close the _SOURCE guard evasion, correct two false claims

Three findings from the orthogonal review pass, all fixed.

1. The ESLint rule's `_SOURCE` provenance fallback was pure identifier-
name matching with no binding check, so `new RegExp(userInput_SOURCE)`
   — a function parameter — sailed past the guard. That is the same
rename-evasion class issue #3410 documents, reopened by the very
fallback meant to complement the structural check. Now bound to the
identifier's actual binding kind: import, require-derived const, or
module-scope const; parameters, `let`/`var`, and unresolvable
bindings fail closed. Four RuleTester cases cover the evasion and
prove the legitimate cross-module case still passes.

2. src/pattern.cts's own header carried the stale pre-correction counts
(12 copies / 17 call sites) while CONTEXT.md and the design doc
   carried the corrected ones (~39 / ~44) — a self-contradiction inside
the PR whose entire purpose is deleting divergent copies. Rewritten,
preserving the durable lesson: a named-function census cannot see
inline copies; only a shape-matching guard can.

3. The claim that all deleted copies threw TypeError on non-string was
false. phase-id.cts's copy — the one with 8 external importers — did
String(value).replace(...) and never threw. The seam's locked
signature does not coerce, so this is a real, now-disclosed behavior
change rather than the pure preservation the tests asserted. Audited
all 32 invocations across the 8 importers and 6 in-file callers:
every one is safe by construction (upstream truthy guard or a
string-producing derivation), verified by runtime probe against the
compiled modules rather than by TS compilation, which cannot see a
runtime undefined. Corrected the false claim in both the test comment
and the design doc, and added it to Known limits.

* docs(#3412): add Changed changeset for the Node 24 floor

The only user-visible break in this phase. The escape-behavior change
is internal and match-equivalent, so it carries no user-facing note.

* fix(#3412): resolve the seam's require graph in script fixtures and packaging

Checkpoint 2 came back red with 90 failures on the node24 lane. Three
distinct defects, all introduced by routing scripts/ through the new
pattern seam, none reproducible by any local gate:

1. ~82 failures — tests/adr-index-gate.test.cjs and
   tests/removed-but-needed-lint.test.cjs copy a scripts/*.cjs into an
mkdtemp fixture and spawn it there (necessary: those scripts resolve
their scan root from __dirname/.., so running the real script would
scan the real repo). Each harness hand-listed the dependencies to
   copy alongside. Adding require('../gsd-core/bin/lib/pattern.cjs') to
gen-adr-index.cjs made both lists silently incomplete ->
MODULE_NOT_FOUND, plus 17 downstream 'did not emit parseable JSON'
   failures from the same crash.

Fixed as a class, not an instance: new tests/helpers/copy-script-
   fixture.cjs walks a script's transitive static relative-require graph
and copies it, so dependencies are derived and never re-declared. It
throws (naming the unbuilt artifact) instead of letting the child die
with a bare MODULE_NOT_FOUND. Verified for all four seam-consuming
   scripts: gen-adr-index, lint-removed-but-needed, gen-loop-host-
   contract, sync-runtime-launcher.

2. 2 failures — scripts/ ships wholesale but eslint-rules/ does not, so
   the new scripts/lint-no-adhoc-regex-escape.cjs would be
MODULE_NOT_FOUND in a published install (#2858 guard). Excluded from
the tarball, matching the existing precedent for gen-emitted-
baseline.cjs, which is excluded for the identical reason, and locked
with a test modeled on that one. Confirmed against a real npm pack:
   890 files, 0 from eslint-rules/, and gsd-core/bin/lib/pattern.cjs
present (so the other four scripts' requires are legitimate).

3. 6 failures — tests/phase-id.test.cjs asserted the literal escaped
   source text ('0*29', 'PROJ-42'). RegExp.escape is match-equivalent to
the retired hand-rolled escaper but NOT text-equivalent: it hex-
escapes the leading character and all hyphens ('0*\x329',
   '\x50ROJ\x2d42'). Verified NOT a behavior change — 576 match
decisions across all three real interpolation prefixes, zero
divergence. Those tests now compile each source into the same heading
regex src/roadmap.cts's searchPhaseInContent builds and assert what
matches and what does not, including the 'i'-flag canonicalization
the hex escape has to preserve. Re-pinning the new literals would
have rebuilt the same brittleness one layer down. Adds a test for the
property the escape exists for: a dot in '1.2' must not act as a
   wildcard.

Also shares one definition of 'a require' between the packaging guard
and the fixture copier, so the two cannot disagree about what they scan.

Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>

* fix(#3412): refuse to copy a fixture dependency outside the fixture root

copyScriptWithDeps resolved each relative require and joined the
repo-relative result onto fixtureRoot. A require resolving OUTSIDE the
repo yields a '../'-prefixed relative path, so path.join climbed out of
the fixture and wrote into the surrounding temp dir (verified:
repoRoot=/repo + depAbs=/etc/passwd wrote /tmp/etc/passwd).

No script in the tree does this today, so this closes an available
escape rather than an active one. Refuses via the existing unresolved-
require path so the failure names the offending specifier. Covered by a
negative proof that the guard fires and that nothing lands outside the
fixture.

* fix(#3412): parse requires instead of pattern-matching them; restore the foreign-prefix contract

Applies all findings from the second orthogonal review round, re-run
because real code changed after round 1.

HIGH (security) — extractRequires stripped BLOCK comments before LINE
comments, so a '//' comment containing '/*' opened a phantom block
comment, and a '//' inside a string literal truncated the line. Both
hid real requires: 'const u="http://x"; require("./real.cjs")'
returned [], and four real requires in gsd-core/bin/gsd-tools.cjs were
invisible. Replaced with a real AST parse via espree.

This is ADR-3212's own Decision 4 — tokenizer-first for stateful
grammars — applied to the case it describes; comment/string/regex
nesting is exactly such a grammar, which is why the regex version was
wrong. The function was moved byte-identical out of the #2858 packaging
guard, so the bug PRE-DATES this branch and has been a live blind spot
there: a shipped script could have required an unshipped path
undetected. Fixing it makes that guard strictly stronger than on next.

espree is promoted from a transitive eslint dependency to an explicit
devDependency rather than relying on hoisting. The script parse attempt
sets ecmaFeatures.globalReturn because Node wraps CommonJS bodies in a
function, making a top-level return legal — scripts/check-coverage-gate
.cjs relies on it, and without the flag the guard throws on a file it
is supposed to scan. Verified 0 unparseable across all 324 .cjs/.js
under scripts/, bin/, and gsd-core/bin/, and 0 new violations against a
real npm pack, so the exact extractor does not newly fail the guard.

MEDIUM (security) — the repo-containment check guarded dependencies but
not the entry path. One escapesContainment predicate now guards both.

LOW (security) — containment was lexical while fs follows symlinks, and
a directory symlink could mint a fresh dedupe key per level. realpath
now resolves both repoRoot and each dependency before the decision, and
the realpath-derived path is the dedupe key. Destination layout still
uses the original repo-relative path, so copied trees are unchanged.

MAJOR (standards) — the round-1 behavioral rewrite of phase-id tests
lost the foreign-prefix contract: every assertion was satisfied by an
impl returning [A-Z]+\x2d42, i.e. ANY project code — the exact #3599
bug class the exact-source prevents. The literal assertions it replaced
were catching this. Now asserts the compiled regex REJECTS a different
prefix with the same number.

MAJOR (standards) — the test hand-duplicated production's heading regex
with no parity guard (CLAUDE.md's 'Generative Fix Divergence'). Removed
the parallel surface instead of policing it: src/roadmap.cts exports
buildPhaseHeadingRegex, searchPhaseInContent calls it, the test imports
it. Byte-identical .source and .flags verified for both escaped forms.

MINOR — '..foo' no longer false-flagged as an escape; the inverted
spurious-vs-missing doc claim corrected; the dead allow-test-rule
header removed.

* chore(#3412): backfill changeset pr number to 3416

* fix(#3412): make the escape guard's own regex linear, reword an injection-scan collision

Two CI failures on PR #3416, both in code this branch added.

CodeQL js/redos (high) — REPLACE_CALL_RE's outer alternation let a
bracket run be consumed EITHER by the character-class branch OR one
character at a time by the trailing catch-all, so a failing match
explored both parses of every pair. Measured on the real regex:
n=26 -> 204ms, n=28 -> 791ms, n=30 -> 3475ms, a clean 2^n. This script
scans repo source, so a file with a long bracket run after '.replace(/'
would hang CI outright — a guard against undisciplined pattern
construction was itself the worst pattern in the diff.

Fixed the way ADR-3212 already prescribes: the catch-all branch now
excludes '[' and ']' so a bracket can only be consumed by the class
branch (this is what makes it linear), and every quantifier is bounded
(the locked bounded-quantifiers decision) as a second line of defense.
Now 0ms at n=2000. Disclosed coverage tradeoff, recorded at the
constant: a regex literal with a BARE unescaped ']' outside a class is
no longer matched by this backstop. No census shape has that form, and
the AST rule remains the primary detector.

Verified the guard did not go blind doing it: a real census-shape
violation is still reported, and an allow-adhoc-regex-escape
suppression comment is still honored.

Regression test drives the exported findViolations on a
2000-repetition adversarial input and asserts the RESULT. It makes no
wall-clock assertion — elapsed-time tests are forbidden — so a
regression surfaces as a harness timeout, which is the correct signal.

Prompt injection scan — 'must not act as a regex wildcard' in a test
comment matched the scanner's jailbreak pattern act\s+as\s+(a|an|if|
my). Reworded to 'behave as'. Deliberately NOT allowlisted: silencing a
whole test file over one phrase would blunt the scanner permanently,
and the comment has nothing to do with injection.

Neither failure was reachable from the remote runner — CodeQL and the
injection scan are not in that matrix, so the sha it passed was green
and still wrong.

1c1af70a4b refactor(#2724): delete the committed golden fixtures and size baselines (#2767)
* test(#2724): delete golden-install-parity fixtures, test, and generator

Removes the 19 committed path->hash manifests, the two per-file size
baselines, tests/golden-install-parity.test.cjs, and
scripts/gen-golden-install-parity-zcode.cjs. These were pure functions
of the source tree (ADR-2719); the differential attribution check
(tests/emitted-attribution.test.cjs + tests/emitted-provenance.test.cjs)
is now the sole gate for emitted-artifact propagation.

tests/fixtures/install-tree/*.json and tests/golden-install-tree.test.cjs
are unchanged (ADR-2719 section 7 exception).

Follow-up commits fix the resulting bookkeeping: scripts/ci-test-scope.cjs's
existence guard, .gitattributes, package.json scripts, the emitted-provenance
totality guard's IO, the differential check's baseline acquisition, CI
wiring to publish/restore the baseline artifact, and docs.

* refactor(#2724): make the differential attribution check self-sufficient

Three fixes required to delete the golden fixtures without breaking CI:

- scripts/ci-test-scope.cjs: remove tests/golden-install-parity.test.cjs
from the three rules that named it. #2759's missingRuleTestFiles guard
hard-throws at module load if a rule names a test file absent from
disk, which would break the changes job on every PR the moment the
  fixture-deletion commit landed.

- tests/helpers/emitted-provenance.cjs: loadManifests() read the
committed golden fixture directory. With that directory deleted at
every future ref, this would throw at module load forever, taking
the Phase 2 totality guard down with it. Rebuilt from real installer
  spawns (MANIFEST_FAMILIES + runMinimalInstall + buildParityManifest),
the same shape emitted-runtime.cjs's currentManifests() already uses.

- tests/emitted-attribution.test.cjs / tests/helpers/emitted-runtime.cjs:
the real-tree test's baseline acquisition swaps from
baselineManifestsAtRef(base) (git show at a ref that no longer carries
fixtures) to resolveBaseline()'s documented precedence: env, then the
on-disk cache, then an in-job build. The build fallback
(buildBaselineAtRef, new) checks out base into a throwaway git
worktree and runs the new scripts/gen-emitted-baseline.cjs there --
no npm ci needed, since bin/install.js and the test helper shells are
Node-builtins-only. That script also publishes the baseline artifact
  from CI's push-to-next job (wired in a follow-up commit).

* refactor(#2724): retire the merge-driver bridge and per-file size baselines

The Phase 1 bridge (#2721) is retired now that the artifacts it guarded
are deleted: scripts/git-merge-regen-driver.cjs, its test, and the
'setup:merge-driver' npm script are removed, and the .gitattributes
merge=gsd-regen/linguist-generated block for the three deleted-path
globs is dropped. tests/fixtures/install-tree/*.json keeps its normal
merge behavior, unchanged (ADR-2719 section 7).

scripts/update-size-baseline.cjs and its test are removed: their sole
purpose was regenerating tests/workflow-size-baseline.json and
tests/agent-size-baseline.json, both deleted. The 'size:baseline' npm
script and its step in 'regen:derived' go with it. The per-file
baseline describe blocks in tests/workflow-size-budget.test.cjs and
tests/agent-size-budget.test.cjs are removed for the same reason; the
independent loose-tier hard caps are untouched. The differential
attribution check's size ratchet (tests/emitted-diff.cjs, already
shipped in #2723) is the replacement anti-creep mechanism.

'npm run gen:golden' is replaced by 'npm run gen:install-tree', which
keeps regenerating tests/fixtures/install-tree/*.json (the one artifact
family ADR-2719 section 7 keeps committed); tests/golden-install-tree.test.cjs's
error messages point at the new command name.

tests/golden-parity-single-source.test.cjs's anti-divergence guard
(#2266) is retargeted from the two deleted golden-parity consumers to
their two replacements (tests/helpers/emitted-runtime.cjs and
tests/helpers/emitted-provenance.cjs), which import buildParityManifest
the same way — the divergence risk the guard exists for is unchanged.

Also wires CI: a new publish-emitted-baseline job runs
scripts/gen-emitted-baseline.cjs after a push to next and caches the
result keyed on the sha; the test and test-full jobs restore that cache
on pull_request events, keyed on the PR's base sha, and export
GSD_EMITTED_BASELINE for tests/emitted-attribution.test.cjs's real-tree
test to pick up.

* docs(#2724): flip ADR-2719 to Accepted and update contributor docs

Status: Proposed -> Accepted. Regenerated docs/adr/README.md index.

CONTRIBUTING.md, docs/TESTING-SUITES.md, and CONTEXT.md (RULESET.
EMITTED_ATTRIBUTION, RULESET.WORKFLOW_SIZE_BUDGET, RULESET.
AGENT_SIZE_BUDGET, and the Emitted Artifact Provenance glossary entry)
no longer point at the deleted golden-install-parity fixtures, size
baselines, gen:golden, UPDATE_GOLDEN, or the setup:merge-driver /
git-merge-regen-driver.cjs bridge. Editing shipped content now
requires zero manual fixture regeneration, documented against the
differential attribution check instead of the deleted commands.

* docs(#2724): add changeset for removed golden-parity commands

* fix(#2724): drop stale scripts/update-size-baseline.cjs glossary ref

check-glossary-refs.cjs verifies every backtick-wrapped scripts/*.cjs
token in CONTEXT.md resolves to a real file. The RULESET.
EMITTED_ATTRIBUTION rewrite named the deleted script inside backticks,
which the checker reads as a live reference, not historical prose.

* test(#2724): retarget ci-test-scope tests off the deleted golden test

tests/ci-test-scope.test.cjs asserted specific RULES entries select
tests/golden-install-parity.test.cjs, and that every rule selecting it
also selects both emitted gates. Both premises broke when the golden
test was deleted (#2724): the deleted filename never re-appears in
targeted_tests, and there was no longer a third file for the gates to
travel alongside. Retargeted the two selection describe blocks to
assert tests/emitted-provenance.test.cjs directly (the drift guard the
golden gate's rules were retargeted to), and simplified the third block
to assert the two emitted gates always travel together, without
reference to the golden filename.

* docs(#2724): repoint two contributor how-to guides at the differential check

Both guides told contributors to regenerate a baseline against
tests/golden-install-parity.test.cjs, which #2724 deletes. Repointed
at the differential attribution check (tests/emitted-attribution.test.cjs,
ADR-2719), which needs no manual regeneration step.

* fix(#2724): repair phase6-capstone-conformance's deleted-baseline read

An independent orthogonal review caught a real regression this branch
introduced into a test file the branch's diff never touched:
tests/phase6-capstone-conformance.test.cjs read
tests/workflow-size-baseline.json (deleted earlier in this branch) with
no fallback, so the whole suite would throw ENOENT the moment this
branch landed. The test's actual intent — prove the host-loop workflow
files are real, tracked, non-empty docs — is preserved by asserting the
live byte count via the same shared counter (scripts/workflow-size.cjs)
the size guards already use, instead of a committed snapshot.

Also, from the same review: a stale doc comment in
scripts/workflow-size.cjs still named the deleted
scripts/update-size-baseline.cjs as a consumer, and
buildBaselineAtRef's cleanup in tests/helpers/emitted-runtime.cjs left
two fs.rmSync calls unguarded against masking the primary result/error,
inconsistent with the try/catch already wrapping the git cleanup beside
them. Both fixed. A doc comment was added to baselineFamilyNamesAtRef
explaining why it (and its siblings) are kept despite having no
production caller post-cutover — they still answer real questions
about refs that predate the cutover.

* fix(#2724): repair three real regressions found by remote verification

1. tests/emitted-provenance.test.cjs's two hostile-input tests
(non-object manifest, unreadable fixture) drove loadManifests(tmp)
and monkeypatched fs.readFileSync, both premised on the deleted
fixture-directory read this branch already replaced with real
installer spawns -- the negative assertions silently stopped firing.
loadManifests() now accepts injected {families, install, build,
clean} (defaulting to production values), giving the tests a real
seam to drive a bad build result and a build failure through the
ACTUAL loader instead of a reimplementation, and added coverage that
clean() still runs on both paths.

2. .github/workflows/test.yml's two 'Export GSD_EMITTED_BASELINE'
steps hardcoded shell: bash, which is wrong on windows-latest (native
pwsh) and on test-full's macos-latest legs (native zsh per that job's
   own matrix) -- the repo's H1 shell policy (tests/policy-shell-pinning
.test.cjs) caught it. Replaced the inline bash script with
   scripts/ci-export-emitted-baseline-env.cjs, a plain Node script: a
bare 'node <path>' command line has no shell-specific syntax, so it
runs correctly under bash, zsh, and pwsh without a shell override.

tests/phase6-capstone-conformance.test.cjs's deleted-baseline read
(caught by the same remote run, at a commit prior to this one) was
already fixed in d0c3b1242 and is not touched here; verified still
passing after these changes.

* fix(#2724): revive ADR-1610's new-file size cap inside the differential

An isolated review caught a real regression: deleting
tests/workflow-size-baseline.json silently dropped NEW_FILE_CAP
(ADR-1610 Decision point 3, the Codex project_doc_max_bytes anchor)
with no successor. tests/helpers/emitted-diff.cjs's size ratchet
already 'continue's past any file absent from sizeBaseline -- exactly
the files this cap exists to bound -- so a brand-new workflow file
sized 32,769-40,960 bytes passed CI clean and shipped, then risked
silent truncation at the Codex anchor at runtime. ADR-1610 is Accepted
and never referenced anywhere in this branch.

Fix: NEW_FILE_CAP=32768 revived inside emitted-diff.cjs's own
size-ratchet loop, keyed off the SAME hasOwnProperty(sizeBaseline,
name) signal the growth check already computes -- 'new' is exactly
'present in sizeCurrent, absent from sizeBaseline'. Not ack-able,
matching the tier hard caps it sits beside: the fix is extraction, not
an acknowledgment entry. Documented, disclosed narrowing: the pure
differential module cannot see XL_WORKFLOWS/LARGE_WORKFLOWS tiering
(tests/workflow-size-budget.test.cjs's classification), so a
legitimately large new file must extract rather than tier in, one
release earlier than an existing file would need to. ADR-1610 itself is
left unamended -- this restores its decision rather than re-litigating
it.

Also fixes a stale comment plus a redundant real 19-installer-spawn
assertion left over from the pre-injection-seam version of
tests/emitted-provenance.test.cjs's build-failure test, and annotates
3 of 4 stale golden-fixture citations in
docs/reference/host-integration-capability-matrix.md as superseded
(the 4th is an accurate historical PR narrative, left alone).

* fix(#2724): repair three red CI defects on the golden-fixture cutover

Windows-only provenance false attribution (defect A): the `hooks-built`
provenance rule attributed `hooks/<name>.cmd` to itself. Those shims are
Windows-only installer output (ensureCodexHooksJsonSessionStart /
ensureCodexHooksJsonEvent, both in src/runtime-hooks-surface.cts) wrapping
the same-named `.js` hook — no `.cmd` file is ever tracked in the repo, so
the self-attribution resolved to a path that exists on no platform. Only
windows-latest ever emits the key, so this only failed there. Fixed by
special-casing `.cmd` inside the SAME `hooks-built` rule (not a dedicated
rule) — a dedicated rule would match zero paths, and therefore report as a
dead rule, on every non-Windows lane of the same totality guard. `sources`
already supported per-match functions; `transforms` is extended to support
the same shape so the attribution can vary by match within one rule.

Baseline bootstrap was structurally impossible (defect B): `buildBaselineAtRef`
ran `scripts/gen-emitted-baseline.cjs` from INSIDE the base-ref worktree, but
that script is new in this PR and therefore absent at any base ref that
predates it — every call failed closed with "Cannot find module". Fixed by
running the PR checkout's own generator against the worktree via a new `--dir`
parameter, decoupling "which copy of the script runs" from "which tree it
measures" (`currentManifests`/`currentSizes` gained a `repoRoot` override,
threaded down to `runMinimalInstall`'s new `installScript` override). This is
not just a bootstrap fix: a differential needs ONE measurement schema applied
to both sides, or the two stop being comparable the moment that schema
evolves — running each side's own copy would silently reintroduce that risk.
Verified locally end-to-end against real origin/next: resolves a valid
{version, sha, manifests, sizes} artifact with the correct sha and no leaked
worktree.

Changeset placeholder (defect C): `pr: 0` -> `pr: 2767`, which is what let
docs-lint evaluate the fragment for the first time; it already passes
(docs/TESTING-SUITES.md and friends already document the removed scripts).

Also fixed while in this file: an eslint no-unused-vars warning surfaced by
the changed lint run (unused `cleanup` import in
tests/emitted-provenance.test.cjs).

Added regression coverage for both A and B: a cross-platform spot-check that
drives the real hooks-built rule against `.cmd` keys directly (not through a
real Windows install), and a real-tree test that drives buildBaselineAtRef
against a base ref verified (via git cat-file) to lack the generator, both
skipping honestly rather than false-passing when their precondition does not
hold.

* fix(#2724): repair false .cmd byte-provenance and a permanently-skipping regression test

Two isolated-review findings on PR #2767:

- `hooks-built`'s `.cmd` branch attributed the Windows shim's bytes to the
wrapped `hooks/<name>.js` script, asserting a byte-provenance link that
does not exist — traced against buildCodexHookWindowsShimIR
(src/runtime-hooks-surface.cts), only the script's NAME (a literal in that
same file) flows into the .cmd bytes, never its content. Point `sources`
at HOOKS_WINDOWS_SHIM_SRC instead, matching the code-derived convention
used elsewhere in the table. Since `sources` is checked before
`transforms` in the differential, the wrong mapping silently excused any
.cmd byte movement caused by editing the wrapped .js file.

- The `buildBaselineAtRef` regression test skipped unless a resolvable base
ref still lacked scripts/gen-emitted-baseline.cjs — true only until this
PR merges, after which every base ref carries the file and the test skips
forever with zero ongoing coverage. Rebuilt hermetically: synthesize the
missing-generator condition in-place via git plumbing (a throwaway commit,
child of HEAD, with just that one file removed from a scratch index),
never touching the real working tree, HEAD, or index, and never depending
  on ambient history or remotes.

* fix(#2724): tolerate the remote runner's dubious-ownership git mount in the emitted baseline path

The runner container mounts the repo at a path owned by a different uid than
the process running the suite, so git's dubious-ownership protection refuses
every git operation there. GitHub Actions never hits this because
actions/checkout registers the workspace as safe automatically; this
runner's container does not.

buildBaselineAtRef is the production build-fallback the sole remaining
emitted gate depends on (resolveBaseline's in-job-build leg), not just a
test helper, so the fix is in the shared git() wrapper (emitted-runtime.cjs)
that every caller — resolveChangedPaths, resolveBase, buildBaselineAtRef's
worktree add/remove/prune, and the hermetic regression test added in the
prior commit — funnels through, plus gen-emitted-baseline.cjs's own
rev-parse (now reusing that same wrapper instead of a second execFileSync,
so the fix has one source of truth). Each call declares -c
safe.directory=<the exact directory it already operates on>, never the *
wildcard.

Audited every other helper on this surface (emitted-diff.cjs,
emitted-baseline.cjs, install-shared.cjs) for the same gap: none of them
shell out to git at all.

d04592de58 fix(#2758): select the emitted differential wherever golden-parity runs (#2759)
* fix(#2758): select the emitted differential wherever golden-parity runs

Add tests/emitted-provenance.test.cjs and tests/emitted-attribution.test.cjs
to every scripts/ci-test-scope.cjs rule that already selects
tests/golden-install-parity.test.cjs, so the ADR-2719 dual-run differential
travels with the golden on the targeted CI lane instead of being selected by
no rule at all.

Add an independent module-load totality guard (missingRuleTestFiles) that
throws when any rule names a test file absent from disk -- the guard that
would have caught the post-Phase-4-cutover hole. It immediately surfaced
three pre-existing phantom entries left behind by consolidation epic #1969
(bug-3588/bug-10/bug-3683 filenames folded into other suites months ago but
never removed from the rule table); fixed in the same change rather than
deferred.

* fix(#2758): trim unused exports and normalize the path require style

Code-review (Standards axis) flagged two judgement-call smells: exporting
classify/isInertCi with no caller (Speculative Generality), and requiring
path with a node: prefix while the file's other core requires do not
(inconsistent style within one file). Both addressed.

89b1bef881 refactor(#2267): golden-parity file-set snapshot + anti-staleness CI selection (#2274)
Phase 2 of golden-parity redesign (epic #2264). Adds an install file-set snapshot (golden-install-tree) and a ci-test-scope rule selecting golden-parity whenever any installed-source path changes, closing the silent-staleness hole behind the #2266 red. ADR-2264 amended (the copy/transform split premise was unsound). Closes #2267.

241b08fa18 ci(#1975): exclude release-tarball-smoke from scoped lane (fixes Windows chunk timeout)
This consolidation PR's breadth (28 changed test files) exposed a scoped-test-lane
capacity limit: ci-test-scope pulls the 3–6 min release-tarball-smoke.install.test.cjs
(npm pack + npm install -g, 10MB/1499 files) into the targeted+windows lane whenever
install files change AND when it is itself a changed file — bundling it with the other
27 files overran the 600s per-chunk timeout on windows-latest-24 (deterministic).

release-tarball-smoke has its OWN dedicated workflow (.github/workflows/install-smoke.yml,
triggered on the production install paths), so its scoped-lane run is redundant. Add a
SCOPED_LANE_EXCLUDE guard that drops it from both targeted_tests and windows_tests however
it entered (matched rule OR changed-file), and remove it from the install rule's tests list.
Update the ci-test-scope.test.cjs assertion accordingly. No coverage lost.

4525bc6f4d fix(#1749): close epic #1702 audit gaps — drift-guard bin/install.js, ci-test-scope wiring, ADR divergence (#1751)
Post-merge coverage-audit follow-ups to epic #1702 (found by an independent
gpt-5.5/high audit + adr-phase-coverage cross-reference). None are CRITICAL —
the 9-rule enforcement shipped and works; these close completeness/integrity
gaps between ADR-1703's promises and the as-built reality.

1. drift-guard bin/install.js scope (ADR-1703 L114-119): the drift guard
covered src/runtime-homes.cts only; the ADR named bin/install.js too. Phase
6's glob expansion made bin/install.js a covered surface. Extended
tests/portability-vocab-drift.test.cjs with TWO sound checks: (a) any
bin/install.js top-level function that directly returns path.*() must be in
PATH_RETURNING_FNS (tight, 0 FP — the body-contains heuristic is unsound
here, ~33 FPs); (b) a curated two-way existence lock on the installer path
helpers (catches a rename making a vocab entry stale; keeps the curation in
sync with PATH_RETURNING_FNS). The residual new-resolver boundary (temp-var
   shape) is documented.

2. ci-test-scope wiring (the Phase 6 portability selection rule was
ineffective): eslint-rules/ was not in the product-code prefix list, so an
eslint-rules-only change set code_changed=false and CLEARED the matched
tests (reproduced: targeted_tests=[]). Added eslint-rules/ to the prefix
list and the P1-P4 RuleTester suites to the selection rule (it previously
listed only P5/P6). Verified: code_changed=true, 11 tests selected.

3. ADR-1703 acceptance note amended to record the two further as-built
divergences: the disable-ban shipped as an out-of-band test (not the
specified local/no-portability-disable meta-rule — the test runs outside
ESLint so it cannot be self-disabled, at least as strong); and the
   drift-guard bin/install.js scope resolution above.

Epic #1702 all eight phase boxes now checked. No runtime change; no-changelog
(contributor tooling + docs).

871621c3c8 feat(#1740): require-fs-op-fallback production AST rule + Windows transient-lock retry (Phase 6) (#1742)
* feat(#1740): require-fs-op-fallback production AST rule + Windows transient-lock retry (Phase 6)

ADR-1703 Phase 6 of the cross-platform portability epic (#1702). Adds the
second production-code portability AST rule + the ADR-mandated glob expansion
to bin/install.js and scripts/build-hooks.js.

- eslint-rules/require-fs-op-fallback.cjs: flags an unguarded fs.rename /
fs.renameSync (the atomic-publish primitive named first in
DEFECT.WINDOWS-FS-OPS.symptom) that is NOT inside a try/catch whose handler
  references a transient errno ('EPERM'/'EBUSY'/'EACCES' or a *RETRY_ERRNOS
set) AND NOT behind a Windows platform guard. A catch that silently swallows
or cleans-up-and-rethrows without an errno check does NOT satisfy the
defect's 'never silently swallow' clause. copyFile/unlink are deliberately
not flagged (they are the fallback primitives per the defect's own
  fix-forward). Scope narrowed to rename per Phase 5's precision discipline;
  documented on #1740.

- src/shell-command-projection.cts: export retryRenameSync(from, to) — the
drop-in bounded-retry helper over the existing atomicRenameWithRetry.

- 27 bare fs.renameSync sites across 11 modules routed through retryRenameSync
  (capability-lifecycle/lock/source, installer-migrations, milestone, phase,
  planning-workspace, roadmap-upgrade, runtime-hooks-surface, state,
workstream). Idempotent on POSIX; resilient to AV/indexer transient locks
  on Windows.

- eslint.config.mjs: register rule at error on src/**/*.cts; new focused
  portability-rules block covering bin/install.js + scripts/build-hooks.js
(ADR-1703 L124-126 glob expansion — both files are compliant: zero
  rename violations).

- tests: 15-case RuleTester suite; portability-rule-disable-ban extended
  (PROTECTED_RULES + scans bin/install.js/build-hooks.js with shebang
  handling); ci-test-scope portability-lint selection rule.

- CONTEXT.md DEFECT.WINDOWS-FS-OPS predicate rewritten to point at the rule;
  docs/contributing/cross-platform-portability-rules.md reference + how-to.

Closes #1740

* chore(#1740): backfill changeset pr:1742

* fix(#1740): tighten require-fs-op-fallback precision (codex review HIGH-1/HIGH-2)

Addresses two false-negative findings from the codex (gpt-5.5/high)
adversarial review of PR #1742:

HIGH-1 — a catch that REFERENCES a transient errno but only rethrows (no
retry/fallback) was marked compliant. The DEFECT.WINDOWS-FS-OPS fix-forward
requires retry, not just recognition. Fix: catchHandlerHasRetrySignal now
requires a loop `continue` backedge OR a `return <call>` delegation; a bare
rethrow is flagged. The misleading `/* retry logic */` valid test is replaced
with a real retry loop, and the rethrow-only shape is added as invalid.

HIGH-2 — the nested-try ancestor walk treated an OUTER errno-catch as
protecting the rename even when an INNER catch intercepted/swallowed the error
(the outer catch is unreachable). Fix: isInsideTransientErrnoTryCatch now stops
at the NEAREST enclosing TryStatement WITH A CATCH HANDLER whose block contains
the rename (try-finally is skipped — it doesn't catch); outer catches are no
longer consulted. The unsound nested-try valid test is converted to invalid,
and a try-finally-skipped valid case is added.

Verified: 17 RuleTester cases pass; zero new production violations (the 27
fixed sites use retryRenameSync; the real retry loops — atomicRenameWithRetry,
capability-ledger/consent, build-hooks — remain compliant via continue/errno);
lint:ci green; disable-ban + vocab-drift green.

9d52043f50 feat(#1733): normalize-path-in-content production AST rule + fix Windows agent-skills content leak (Phase 5) (#1736)
* feat(#1733): normalize-path-in-content production AST rule (Phase 5)

ADR-1703 Phase 5 — the first production-code rule. local/normalize-path-in-content
(src/**/*.cts, @typescript-eslint/parser): flags a path-returning fn result
(path.basename excluded — returns a separator-less filename) interpolated into an
@-reference / config-dir markdown body without .replace(/\\/g,'/') normalization,
per RULESET.CONTENT-PATH-NORMALIZATION / DEFECT.WINDOWS-PATH-LEAK-IN-MARKDOWN-CONTENT.

Build-and-assess found the canonical defect site (computePathPrefix) already
compliant and only 1 src/ hit — a false positive (path.basename in a status
message) — eliminated by narrowing (exclude basename; require a real @-ref/
config-dir marker, not bare .md). 0 src/ violations: clean forward-prevention.

The out-of-band disable-ban now scans src/**/*.cts too (typescript-estree) so the
production rule also cannot be eslint-disabled. Registered (error) + PROTECTED_RULES;
CONTEXT.md predicates + how-to doc updated.

- RuleTester suite (26 cases)

* fix: harden mutation-matrix.cjs stdin read against EAGAIN on non-blocking pipe

scripts/mutation-matrix.cjs read piped stdin via readFileSync(process.stdin.fd).
On macOS libuv marks the stdin pipe fd non-blocking, so a synchronous read can
throw EAGAIN before the writer fills the pipe — intermittently, under heavy CI
shard load — aborting the script (status 2) and flaking mutation-matrix-ratchet.
Replace with readStdinSync(): an fs.readSync loop that retries on EAGAIN (1ms
synchronous Atomics.wait yield), stops on 0-byte/EOF, and rethrows other errors.
Deterministic regression test injects EAGAIN via an fs.readSync monkeypatch.

* ci: re-run golden-install-parity on src/lib + installer changes (close drift guard)

golden-install-parity hashes every installed bin/lib/*.cjs per runtime, so it
must re-run whenever the built lib could change. ci-test-scope selected it for
neither src/** nor installer changes, so a source-only edit (e.g. #1691's
milestone.cts/roadmap.cts) recompiled bin/lib and silently drifted the golden
fixtures past the scoped lane. Add golden-install-parity.test.cjs to both the
'TS runtime sources' and 'installer and package layout' selection rules, with
behavioral regression tests for each.

a647053dcf ci(scope): narrow #494 invariant — changed tests join windows lane, not full matrix
full_matrix fired on 15/15 sampled PRs because any tests/** change forced it,
costing ~25 runner-minutes each. A changed test file now always joins the
scoped windows lane (covering the #482 OS-specific failure class per-file)
and still runs on ubuntu 22/24 via targeted_tests; the residual macOS /
windows-node-22 cross-product is covered on every push to next.

Also narrows WINDOWS_HINTS from 6 substrings (102/633 files, a ~10-minute
scoped lane) to windows/win32/shell/path — the dropped hints (workflow,
install, hook) are either platform-independent lint tests or already covered
by fullMatrix rules.

606363c416 chore(#846): remove unused PR-size labeler (size/S–XL) workflow (#848)
The PR Gate workflow's only job, size-check, labeled every PR with
size/S–size/XL based on lines changed. Those labels aren't used in any
review, triage, or automation flow, so the workflow was pure noise.

- Delete .github/workflows/pr-gate.yml
- Drop size-check from required status checks in both rulesets so PRs
don't block forever on a check that never reports
- Remove pr-gate.yml from INERT_WORKFLOWS (ci-test-scope.cjs) and the
  knownInert list (ci-test-scope.test.cjs)
- Remove "PR Gate / size-check" from setup-branch-protection.sh

988024c1a3 fix(#837): three-dot diff in ci-test-scope so docs-only PRs skip the heavy matrix (#841)
CI test-scope detection diffed changed files with a two-dot
`git diff --name-only base head`, where base is the moving tip of `next`.
A PR branch cut from a slightly older `next` surfaced every product file
`next` had gained since the merge-base, flipping product_changed/full_matrix
and running the full Windows/macOS matrix + coverage on docs-only PRs.

Switch to a three-dot `git diff --name-only base...head` (vs the merge-base),
matching GitHub's PR "Files changed" semantics. Add a regression test that
builds a stale-base topology, plus a guard test pinning `fetch-depth: 0` on
the `changes` job (required for the merge-base to be locally available).

571d7b5a1c feat(#764): skip cross-platform test matrix for docs-only and inert-CI PRs (#798)
test.yml had no paths filter and the ci-test-scope classifier treated docs/
and every .github/workflows/* as code_changed, so documentation edits and
product-irrelevant automation tweaks still spun up the full Linux/Windows/macOS
matrix. Narrow the heavy matrix to changes that can actually affect the product
or the test pipeline.

- ci-test-scope.cjs: drop docs/ from code_changed (docs-only -> full skip; the
required-tests fan-in still reports green). Add src/ to code_changed (it was
missing -> a source-only PR previously skipped all tests). Add INERT_WORKFLOWS
allowlist + isInertCi() + an "inert CI" rule, and a product_changed output that
gates the heavy test/coverage jobs. Fail-safe: any workflow not on the inert
allowlist defaults to the full matrix. A module-load assertion throws if a
  PROTECTED_WORKFLOWS entry (test/install-smoke/mutation/security-scan/release)
is ever added to the inert set, so a weakening edit fails CI loudly.
- test.yml: keep the static 3-lane matrix (so the H1 shell-policy linter can
still statically verify the Windows lane), gate test/coverage on
product_changed, add a lightweight ubuntu-only test-inert job, and branch the
  required-tests fan-in on product_changed.
- docs-required.yml: run docs-parity-live-registry (gated on docs/ changes) so
pure-docs PRs still catch live-registry drift without the matrix.
- tests: cover docs-only, inert-only, src/, pipeline, unknown-workflow fail-safe,
mixed escalation, the code_changed=false -> no-lanes invariant, and protected-
  workflow tamper-evidence.

f729101eec refactor(scripts): replace process.exit() with ExitError + runMain handler (#739) (#740)
Part 1 of 2 of the n/no-process-exit cleanup (umbrella #738): convert every
process.exit() call in standalone scripts/** CLIs to the rule-compliant pattern.

- New shared helper scripts/lib/cli-exit.cjs: ExitError(code,message) + runMain()
which translates a thrown ExitError / returned number into process.exitCode
(never process.exit()), flushing output and still firing process.on('exit').
- main()-based entrypoints: throw new ExitError(code) for errors, return <code>
for verdicts; invoked via runMain(main). Child exit codes preserved via return.
- top-level-only scripts: imperative body extracted into main() so mid-flow
aborts (throw ExitError) actually halt; pure consts/helpers stay at module scope.
- diff-touches-shipped-paths.cjs: stdin event handling restructured to an async
read so the whole flow runs under runMain; uncaughtException/unhandledRejection
nets replaced by an in-band catch that preserves EXIT_ERROR=2.

Exit codes verified unchanged for every converted script (success/error/help and
the 0/1/2 semantic codes in diff-touches). Rule stays warn here; flipped to error
in part 2 (#738) once gsd-core/bin/** is also clean.

cdd78bd2aa fix(#670): self-healing recovery for installer-migration checksum drift (#675)
Editing the body of an already-released installer migration drifts its computed
checksum (it hashes plan.toString()). The integrity guard then hard-aborted
every prior install on upgrade with "applied migration checksum changed" — a
100% reproducible blocker (v1.3.0, all platforms).

Already-applied migrations are filtered out of `pending` and never re-run, so
a drifted checksum is functionally inert. ADR-0008 anticipates checksum-mismatch
state as something the install-state layer must handle gracefully (plan -> apply
-> recover/report), not abort on.

This supersedes the published-checksum allowlist merged in #674 (per-release
maintenance debt — every historical checksum hand-pinned, still throws for any
unregistered value) with a general, self-healing recovery:

- Replace the throwing guard with non-fatal `collectAppliedChecksumDrift`,
  surfaced on `plan.checksumDrift`.
- Reconcile drifted stored checksums durably on the next state write
  (`reconcileDriftedChecksums`), idempotently (no perpetual writes).
- Relocate the "shipped migration bodies are immutable" rule to a CI baseline
test that locks every shipped migration's checksum and fails on body drift —
where #615 should have been caught, instead of blocking users.

Removes #674's legacyChecksums field, per-migration checksum pins,
published-checksums.json fixture, and compat test.

463cffd894 chore(#604): rename get-shit-done/ runtime directory to gsd-core/ (#615)
* chore(#604): rename get-shit-done/ runtime directory to gsd-core/

Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the
on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary
(`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers
are unaffected.

Mechanical (bulk, ~90% of the diff):
- `git mv get-shit-done gsd-core`
- Swept path/identifier references across the repo via
`perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead
preserves the five legitimate slug variants that are NOT the directory:
  get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names).
- Build/manifest wiring: package.json (bin, files, coverage globs),
  tsconfig.build.json (outDir), ~86 .gitignore build-output entries,
  stryker.config.mjs, scan-ignore files, install.js path strings.
- Frozen (not rewritten): CHANGELOG.md history; translated docs
  (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/).

New logic (review here):
- src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper
ADR-0008 installer migration. On upgrade it walks the legacy
`~/.claude/get-shit-done/` tree, classifies each file via the prior install
manifest, and emits remove-managed / backup-and-remove for managed files
while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked
root and symlinked entries; bounds-checks every path under configDir). The
framework rolls back on install failure. Emptied dirs may remain (framework
has no recursive dir-removal primitive) — documented.
- scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare
`get-shit-done` directory token (split token to avoid self-match; case-
insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists
CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines).
Wired into the lint-tests CI job.
- Restored scripts/lint-package-identity-drift.cjs detection regexes (the
mechanical sweep had wrongly rewritten the old-name patterns it exists to
detect) and marked them as intentional legacy references.
- TDD tests for the migration and the guard; do.md slash-command guard regex
  tightened so a `/gsd-core/bin` path segment is not mistaken for a command;
  changeset + docs/installer-migrations.md row added.

Breaking: the installed runtime path moves `~/.claude/get-shit-done/` ->
`~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed
files (preserving user files) on upgrade. Users with custom hooks/configs
hardcoding the old path must update them.

* fix(#604): unsweep pending changesets + allowlist injection-example docs

CI fixes for the rename PR:
- Do not sweep pending .changeset/*.md (ephemeral release-note fragments,
like CHANGELOG); reverted those body edits so 5 pre-existing malformed
fragments (missing type/pr) no longer enter the PR diff and trip docs-lint.
Allowlisted .changeset/ in the legacy-name guard accordingly.
- Allowlisted TEST-EXAMPLES.md and docs/explanation/security-model.md in
prompt-injection-scan.sh: they contain intentional injection examples /
security-model prose; the path-reference rewrites are kept.

CodeQL alerts on this PR are pre-existing (alert lines unchanged by this PR;
none in the new migration/guard) and are out of scope for the rename.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): resolve CodeQL alerts surfaced on this PR

The rename diff touched files carrying pre-existing CodeQL findings; per the
no-pre-existing-dismissal rule, fixing every surfaced alert rather than waving
them off. All behavior-preserving:

- scripts/ci-test-scope.cjs: build the config-path match from string
.includes() instead of a RegExp over an arg-derived value (js/regex-injection).
- src/profile-output.cts: escape backslashes before pipe-escaping desc/safeName
so the table-cell escape is complete (js/incomplete-sanitization).
- tests/{bug-2643,bug-2808,docs-parity-live-registry}: two-pass HTML-comment
  strip so a bare/unclosed `<!--` cannot survive (js/incomplete-multi-character-sanitization).
- tests/inline-plan-threshold: drop the no-op `\s`->`\s` identity replace,
keep the meaningful POSIX-class conversion (js/identity-replacement).

Verified: build:lib green; the touched test files + ci-test-scope + profile-output
suites pass; lint:legacy-name clean.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): correctly resolve remaining CodeQL alerts (regex-injection + sanitization)

The prior commit's fixes for two alerts were ineffective:
- ci-test-scope.cjs js/regex-injection: the alert is the CLI-arg-derived `file`
reaching static regex `.test(file)` calls (not the config rule). Removed ALL
regex over file/t — startsWith/includes/=== string checks + an isWindowsHint
helper — so there is no regex sink for the tainted value.
- js/incomplete-multi-character-sanitization (3 test files): a single
  `.replace(/<!--...-->/g,'')` can let `<!--` re-form. Replaced with a fixpoint
loop (replace until stable) plus a final bare-opener strip.

Verified: no regex over file/t remains; ci-test-scope + the 3 test suites pass;
lint:legacy-name clean.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): make ci-test-scope + comment-strippers regex-free to clear CodeQL

CodeQL flags the regex PATTERNS syntactically (regex-injection on the
--files arg split; incomplete-multi-character-sanitization on the <!--...-->
replace), so loop fixes do not satisfy it. Made these paths regex-free:
- ci-test-scope.cjs splitFiles: char-by-char separator tokenizer (no /[,\\s]+/).
- 3 test files: indexOf/slice HTML-comment stripper (no .replace(/<!--/)).
Behavior preserved; ci-test-scope + the 3 suites pass; guard clean.

* fix(#604): unblock security base64 scan on the large rename diff

The security job hit its 10m timeout: base64-scan.sh choked on the binary
test fixture tests/feat-3594-parser-property-style.test.cjs (embedded NUL/
non-UTF8 bytes -> thousands of bogus blobs + "ignored null byte" warnings),
and the ~800-file rename diff is slow to scan regardless.

- scripts/base64-scan.sh: skip binary-by-content files (grep -Iq .) — they
can't carry base64-obfuscated *text* and feeding NUL bytes through the
per-line scanner is pathologically slow. collect_files already filtered
binary *extensions*; this catches binary *content* in text extensions.
- .github/workflows/security-scan.yml: raise the security job timeout 10m->30m
to accommodate very large diffs (the scan itself is unchanged).

Verified locally: scan skips the fixture, 0 "ignored null byte" warnings,
0 findings, exit 0.

* fix(#604): sweep get-shit-done refs introduced by merging next

The branch was updated with next (#614/#384/#618 etc.), which reference the
get-shit-done/ dir (still named that on next). Swept the stale references in
the merged files to gsd-core so the rename stays consistent and lint:legacy-name
passes:
- commands/gsd/discuss-phase.md (runtime-launcher shim paths)
- src/core.cts (getAgentsDir layout comments)
- tests/bug-384-agents-runtime-aware.test.cjs (require path to runtime lib)

Verified: guard 0 violations; build green.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): exclude gsd-core/ path segments from bug-3683 command cross-ref invariant

The #614 runtime-launcher shim added to discuss-phase.md references
`${_GSD_RUNTIME_ROOT}/gsd-core/bin/...`. bug-3683's REF_PATTERN excluded path-y
refs only via lookbehind, but `}` precedes `/gsd-core/` in the shim, so it
mis-read the directory path as a dangling `/gsd-core` command ref (same class as
the #604 bug-2954 fix). Added a trailing `(?![\w-]*\/)` so `/gsd-<x>/...` path
segments are not treated as slash-command references.

Verified locally on BOTH platforms before pushing:
- mac (node 26) full suite: 0 failures
- gsd-test-runner (linux, node22 image) full suite: 0 failures
- bug-3683 + bug-2954 pass.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): lazily resolve findProjectRoot in gsd-tools (harden flaky CI)

CI intermittently failed state.test's gsd-tools subprocess with
"findProjectRoot is not a function" (flip-flopping across legs; not reproducible
on mac full suite, gsd-test linux full suite, test:unit, or state.test x8).
findProjectRoot is a re-export from core.cjs (sourced from project-root.cjs);
binding it via destructure at module-load can be undefined under a load-ordering
edge. Resolve it lazily at call time via a small wrapper so the lookup happens
after core.cjs is fully initialized.

Verified green on BOTH platforms before pushing:
- mac (node 26) full suite: 0 failures
- gsd-test-runner (linux, node22) full suite: 0 failures
- state.test.cjs: 106/106; gsd-tools loads cleanly.

* fix(#604): allowlist verification-patterns.md placeholder examples in secret scan

The rename git-mv'd references/verification-patterns.md into gsd-core/, pulling
it into the secret-scan diff. It documents stub/placeholder RED-FLAG env-var
examples (illustrative Stripe test-key / database-URL / API-key placeholders) —
not real credentials. Added it to .secretscanignore with the strict annotation,
mirroring the existing gsd-core/workflows/plan-phase.md exception.

Verified locally: secret-scan-lint --strict OK; secret-scan --diff origin/next
exits 0 with 0 findings.

5cd52eb151 enhancement(#537): pilot TS build-at-publish for bin/lib (semver-compare) (#541)
* docs(#457): rewrite ADR-457 to ground truth and accept build-at-publish

The prior draft asserted a codebase state that never existed (13 tsc-generated
files, src/ trees, a tests/cjs-ts-parity.test.cjs). Corrected to verified ground
truth (84 bin/lib .cjs, 1 value-baked package-identity.cjs, no tsc pipeline),
distinguished value-baking from transpilation so package-identity stops being
miscited as precedent, made check-in-the-artifact vs build-at-publish the central
decision, and flipped status to Accepted (build-at-publish).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* build(#537): pilot TS build-at-publish for bin/lib (semver-compare)

First hand-written module collapsed to a TypeScript source of truth per ADR-457.
src/semver-compare.cts compiles (tsc, strict, noEmitOnError) to a gitignored
get-shit-done/bin/lib/semver-compare.cjs. build:lib is wired into build, pretest,
pretest:coverage, and prepublishOnly so the artifact is built before test and
shipped on publish. Type-aware ESLint on src/**/*.cts immediately caught the
params were over-typed as `unknown` (no-base-to-string); narrowed to a honest
VersionInput domain type. Behavior preserved: semver-compare.test.cjs (14) and
bug-10 (4) pass against the generated output; runtime consumer changeset/cli.cjs
unaffected.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#537): make build-at-publish robust across all CI paths (codex review)

Adversarial review found the pilot's generated artifact would be missing on
clean CI checkouts. `pretest`/`pretest:coverage` only fire for `npm test`, but
CI runs `test:unit`/`test:integration`/`test:install` and `node run-tests.cjs`
directly — none of which built the artifact, so any suite requiring
semver-compare.cjs would hit module-not-found on a clean checkout, and
install-smoke's `npm pack` could ship without it.

- Add a `prepare` script (`npm run build:lib`). `npm ci` runs it automatically,
so every CI test job and install-smoke's pack emit the artifact before use.
This is the idiomatic npm mechanism for compiled-output-not-in-git and fixes
both the test and pack paths in one place.
- Add `src/` + `tsconfig.build.json` to ci-test-scope and the install-smoke /
mutation path filters, so a source-only edit to a migrated module still
triggers its tests and mutation coverage (prevents silent CI skips).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#537): map src/*.cts to built artifact in mutation changed-files detection

Follow-up to the codex re-review. The prior commit added src/**/*.cts to the
mutation workflow's path trigger but left its "compute changed core lib files"
step diffing only get-shit-done/bin/lib/**/*.cjs — which are now gitignored and
never appear in a diff. A source-only edit would trigger the workflow then
early-exit ("no core lib files changed"), silently skipping mutation testing.

Map each changed src/*.cts to its built get-shit-done/bin/lib/*.cjs path (the
on-disk artifact Stryker mutates after prepare/build:lib), merge with the
hand-written .cjs diff, and apply the test/excluded-module filters once.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#537): use 'src/' pathspec in mutation diff (git glob doesn't match top-level)

Codex review caught that `git diff -- 'src/**/*.cts'` returns empty for a
top-level file like src/semver-compare.cts — git's default pathspec glob does
not match `**` across zero directories (verified on git 2.50.1). The prior
commit's src-detection therefore never fired, so source-only changes still
skipped mutation. Switch to the dir-scoped pathspec 'src/' + a `.cts` grep
(robust for flat and nested layouts), and broaden the workflow path trigger to
'src/**' to match install-smoke. Verified end-to-end: a change to
src/semver-compare.cts now resolves to get-shit-done/bin/lib/semver-compare.cjs
in the --mutate list.

* chore(#537): add changeset fragment for build-at-publish pilot

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#537): replace prepare with prepack + build-if-missing; defer mutation wiring

CI surfaced three real issues the local run and codex review missed:

1. lockfile-sync failed on every platform. Root cause: `npm ci --dry-run` (the
repo's lockfile health check) RUNS the `prepare` script, but in dry-run the
devDependencies aren't installed, so `tsc` is not found (exit 127) and the
check reports a misleading "out of sync". `prepare` is the wrong hook for a
build needing a devDep. Replace it with `prepack` (runs only on pack/publish,
when node_modules exists) for the tarball path, and build the artifact inside
scripts/run-tests.cjs (build-if-missing) for the test path — the universal
chokepoint every CI test invocation funnels through, including the direct
`node run-tests.cjs --files-from` step that bypasses npm lifecycle hooks. The
guard is a no-op once built, so the run-tests harness test is unaffected.

2. The Stryker mutation gate ran only 1 test against semver-compare (~0% score,
71/71 mutants surviving) — a Stryker test-selection problem orthogonal to the
build migration, and raising the score needs property tests (ADR-456). Revert
the mutation.yml src wiring; mutation coverage for src-authored modules is a
separate follow-up tracked in #537. (The deletion of the gitignored top-level
.cjs does not match the workflow's `bin/lib/**/*.cjs` git pathspec, so the
   gate skips cleanly.)

Verified: clean-room `npm ci --dry-run` exits 0; deleting the artifact then
running a suite rebuilds it; run-tests harness 22/22 green; `npm pack` includes
the built artifact via prepack.

48b1e35187 fix(#431): enforce H1 shell policy (linux=bash, macOS=zsh, windows=pwsh) across PR + release gates (#434)
* test(#431): policy-shell-pinning linter — RED baseline (37 violations on origin/next)

Adds scripts/workflow-policy.cjs: H1 shell-policy linter with POLICY map,
VIOLATION enum, matrix expansion, effective-shell resolution order, and
runPolicyLint({ workflowsDir }) entry point.

Adds tests/policy-shell-pinning.test.cjs: 8 tests (baseline + 6 synthetic
counter-tests). Synthetic tests 2–7 pass; baseline test is intentionally RED
(37 violations: 28 in test.yml, 9 in install-smoke.yml — all macos/windows
lanes using shell: bash instead of native zsh/pwsh).

Adds js-yaml@4.1.1 as devDependency for YAML parsing.

* fix(#431): switch ubuntu/windows lanes to native shells; extract bash-isms to Node

Remove all explicit shell: bash pins from ubuntu-only jobs (changes, lint-tests,
coverage, required-tests, smoke-unpacked) — ubuntu runner default is bash, which
is both H1-compliant and the runner default, making the pin redundant.

For the test and test-full mixed-OS jobs (ubuntu+windows, windows+macos):
- Move bash-ism steps to shell-agnostic Node scripts:
    scripts/ci-guard-runner.cjs — RUNNER_ENVIRONMENT check
    scripts/ci-rebase-check.cjs — git fetch+merge PR base branch
    scripts/check-npm-integrity.cjs — Node port of check-npm-integrity.sh
    scripts/ci-prepare-test-scope.cjs — write .ci-selected-tests.txt
    scripts/ci-smoke-skip.cjs — set skip= output for full-only matrix entries
- Remove shell: bash from simple npm/node command steps (runner default applies)

This brings Windows violations from 19 to 0. Remaining 17 violations are all
MACOS_MISSING_EXPLICIT_ZSH in mixed-OS matrix jobs (test-full: windows+macos,
install-smoke smoke: ubuntu+macos) — these require job splitting to fix; see
BLOCKER in PR description.

* fix(#431): update workflow-shell-pinning test for H1 policy

The old test required all Windows-targeting npm steps to pin shell: bash
(to prevent pwsh stderr-swallow). Under H1, Windows runners must use
pwsh (native, no pin needed) — shell: bash on Windows is now the
violation, not the fix.

Update findViolations() to flag npm steps with effectiveShell === 'bash'
(rather than effectiveShell === null). Update synthetic tests to verify
the H1-inverted semantics: defaults.run.shell: bash on Windows is now 2
violations, not 0. Update test name and assertion messages to describe
the H1 constraint rather than the old missing-pin constraint.

* fix(#431): extend policy linter to resolve matrix.shell expressions

- expandRunsOn now captures all matrix.include row keys as realization
context (os, node-version, shell, full_only, etc.) instead of only os
- effectiveShell now accepts a realizationContext and resolves
${{ matrix.<key> }} expressions against it before checking policy
- Unresolvable matrix key in shell expression emits UNRESOLVABLE_MATRIX
- Add 3 new tests: positive (zsh+pwsh per row → 0 violations),
counter (bash in macOS row → WRONG_SHELL_FOR_OS), counter (missing
  shell key → UNRESOLVABLE_MATRIX)

* fix(#431): apply matrix.shell pattern to test-full and smoke jobs (clears BLOCKER)

test-full job (test.yml):
- Add shell: pwsh/zsh per matrix.include row (windows-latest→pwsh,
  macos-latest→zsh)
- Add job-level defaults.run.shell: ${{ matrix.shell }}
- No step-level shell pins existed to remove

smoke job (install-smoke.yml):
- Add shell: bash/zsh per matrix.include row (ubuntu→bash, macos→zsh)
- Add job-level defaults.run.shell: ${{ matrix.shell }}
- No step-level shell pins existed to remove

Policy linter now reports 0 violations across all workflow files.

* refactor(#431): migrate .sh check scripts to .cjs; remove .sh originals

- Add scripts/check-env.cjs: Node.js port of check-env.sh with
identical exit codes (0/1/2), human-readable and --json output,
--help flag, and all 5 checks (node-version, npm-version,
  lockfile-present, lockfile-sync, version-manager-pin)
- Migrate all callers:
  - package.json check:env → node scripts/check-env.cjs
  - package.json check:integrity → node scripts/check-npm-integrity.cjs
  - scripts/ci-test-scope.cjs path strings → .cjs equivalents
  - .github/workflows/release.yml rc+finalize jobs → node .cjs (drop chmod+x)
  - .github/workflows/security-scan.yml → node .cjs (drop chmod+x)
  - tests/check-env.test.cjs → spawn node process.execPath [.cjs]
  - tests/npm-integrity-gate.test.cjs → spawn node process.execPath [.cjs]
- Delete scripts/check-env.sh and scripts/check-npm-integrity.sh

* refactor(#431): update doc references from .sh to .cjs

Update SECURITY.md and docs/contributing/bootstrap.md to reference the
canonical Node invocation instead of the removed bash scripts.

* fix(#431): use per-step shell:matrix.shell instead of defaults.run.shell (GHA compat)

GHA does not reliably resolve matrix expressions inside defaults.run.shell.
Per-step shell: always resolves correctly. Removed the defaults.run.shell block
from the test-full job (test.yml) and the smoke job (install-smoke.yml), and
added shell: \${{ matrix.shell }} directly on every run: step in both jobs.

Codex finding: defaults.run.shell with matrix expressions is not a
GHA-supported pattern; per-step shell: is the safe form.

* fix(#431): policy linter validates every matrix.include row independently

Removed runner-label-only dedup from expandRunsOn() in workflow-policy.cjs.
The prior guard (if !realizations.find(r => r.runner === runner)) collapsed
two macos-latest rows with different node-version/shell contexts into one,
hiding the second row's policy violation.

Each matrix.include row is a distinct CI realization with its own context;
validating it twice is harmless but skipping it causes false negatives.

Added counter-test (Test 8) in tests/policy-shell-pinning.test.cjs:
two macos-latest rows (shell:zsh compliant + shell:bash violation) must
produce exactly one WRONG_SHELL_FOR_OS violation on the second row.

* fix(#431): remove dedup-by-runner in Cartesian matrix.<key> expansion (Codex round 3)

The base-list path in expandRunsOn (matrix.<key> arrays, e.g. matrix.os)
previously guarded each push with `if (!realizations.find(r => r.runner === runner))`,
collapsing duplicate runner values into a single realization and hiding policy
violations on later rows of a Cartesian matrix.

Remove the guard unconditionally; each entry in the base-list array now produces
its own realization, matching the same fix already applied to the matrix.include path.

Add counter-test "Cartesian matrix os × shell — dedup must not collapse rows by
runner alone": matrix.os: [macos-latest, macos-latest] + shell: ${{ matrix.shell }}
now yields 2 realizations (not 1). Documents that Cartesian cross-product expansion
(carrying all keys into realization context) is a separate follow-up; current violations
are UNRESOLVABLE_MATRIX pending that work.

* fix(#431): remove 60s timeout regression on npm ci --dry-run (parity with check-env.sh)

* fix(#431): ci-rebase-check.cjs — return truthy sentinel on success (Codex round 4)

run() used execFileSync with stdio:'inherit', which returns null on success.
Caller checked `result !== null`, always false → every successful fetch fell
through to "failed after 3 attempts" exit-1 path.

Fix: run() now returns true on success, false on failure.
Update caller from `result !== null` to `if (result)`.

Adds tests/ci-rebase-check.test.cjs (5 tests) covering the sentinel contract
and a local-bare-remote integration smoke that verifies the full fetch+merge
path exits 0 when fetch succeeds.

f5f51b5b49 fix(#408): align ci-test-scope smoke handling with #395 changeset (drop unconditional injection; unit fallback) (#420)
- Remove `DEFAULT_SMOKE_TESTS` and `WINDOWS_SMOKE_TESTS` constants (now dead after the unconditional injection block is dropped)
- Drop the `addAll(targeted, DEFAULT_SMOKE_TESTS)` / `addAll(windows, WINDOWS_SMOKE_TESTS)` block from the `codeChanged` branch
- When `codeChanged && targetedTests.length === 0`, push `'unit'` as the fallback suite token
- Two new regression tests in `tests/ci-test-scope.test.cjs` covering the no-injection and unit-fallback contracts (bug #408)
