feat(03-01): create /gsd:analyze-codebase command
- Standalone codebase scanning for brownfield projects
- Uses same extraction patterns as intel-index.js hook
- Produces index.json, conventions.json, summary.md
- Excludes node_modules, dist, build, .git, vendor, coverage
