The prior draft asserted a codebase state that never existed (13 tsc-generated
files, src/ trees, a tests/cjs-ts-parity.test.cjs). Corrected to verified ground
truth (84 bin/lib .cjs, 1 value-baked package-identity.cjs, no tsc pipeline),
distinguished value-baking from transpilation so package-identity stops being
miscited as precedent, made check-in-the-artifact vs build-at-publish the central
decision, and flipped status to Accepted (build-at-publish).

First hand-written module collapsed to a TypeScript source of truth per ADR-457.
src/semver-compare.cts compiles (tsc, strict, noEmitOnError) to a gitignored
get-shit-done/bin/lib/semver-compare.cjs. build:lib is wired into build, pretest,
pretest:coverage, and prepublishOnly so the artifact is built before test and
shipped on publish. Type-aware ESLint on src/**/*.cts immediately caught the
params were over-typed as `unknown` (no-base-to-string); narrowed to a honest
VersionInput domain type. Behavior preserved: semver-compare.test.cjs (14) and
bug-10 (4) pass against the generated output; runtime consumer changeset/cli.cjs
unaffected.

Adversarial review found the pilot's generated artifact would be missing on
clean CI checkouts. `pretest`/`pretest:coverage` only fire for `npm test`, but
CI runs `test:unit`/`test:integration`/`test:install` and `node run-tests.cjs`
directly — none of which built the artifact, so any suite requiring
semver-compare.cjs would hit module-not-found on a clean checkout, and
install-smoke's `npm pack` could ship without it.

- Add a `prepare` script (`npm run build:lib`). `npm ci` runs it automatically,
  so every CI test job and install-smoke's pack emit the artifact before use.
  This is the idiomatic npm mechanism for compiled-output-not-in-git and fixes
  both the test and pack paths in one place.
- Add `src/` + `tsconfig.build.json` to ci-test-scope and the install-smoke /
  mutation path filters, so a source-only edit to a migrated module still
  triggers its tests and mutation coverage (prevents silent CI skips).

Follow-up to the codex re-review. The prior commit added src/**/*.cts to the
mutation workflow's path trigger but left its "compute changed core lib files"
step diffing only get-shit-done/bin/lib/**/*.cjs — which are now gitignored and
never appear in a diff. A source-only edit would trigger the workflow then
early-exit ("no core lib files changed"), silently skipping mutation testing.

Map each changed src/*.cts to its built get-shit-done/bin/lib/*.cjs path (the
on-disk artifact Stryker mutates after prepare/build:lib), merge with the
hand-written .cjs diff, and apply the test/excluded-module filters once.

Codex review caught that `git diff -- 'src/**/*.cts'` returns empty for a
top-level file like src/semver-compare.cts — git's default pathspec glob does
not match `**` across zero directories (verified on git 2.50.1). The prior
commit's src-detection therefore never fired, so source-only changes still
skipped mutation. Switch to the dir-scoped pathspec 'src/' + a `.cts` grep
(robust for flat and nested layouts), and broaden the workflow path trigger to
'src/**' to match install-smoke. Verified end-to-end: a change to
src/semver-compare.cts now resolves to get-shit-done/bin/lib/semver-compare.cjs
in the --mutate list.

* fix(#537): replace prepare with prepack + build-if-missing; defer mutation wiring

CI surfaced three real issues the local run and codex review missed:

1. lockfile-sync failed on every platform. Root cause: `npm ci --dry-run` (the
   repo's lockfile health check) RUNS the `prepare` script, but in dry-run the
   devDependencies aren't installed, so `tsc` is not found (exit 127) and the
   check reports a misleading "out of sync". `prepare` is the wrong hook for a
   build needing a devDep. Replace it with `prepack` (runs only on pack/publish,
   when node_modules exists) for the tarball path, and build the artifact inside
   scripts/run-tests.cjs (build-if-missing) for the test path — the universal
   chokepoint every CI test invocation funnels through, including the direct
   `node run-tests.cjs --files-from` step that bypasses npm lifecycle hooks. The
   guard is a no-op once built, so the run-tests harness test is unaffected.

2. The Stryker mutation gate ran only 1 test against semver-compare (~0% score,
   71/71 mutants surviving) — a Stryker test-selection problem orthogonal to the
   build migration, and raising the score needs property tests (ADR-456). Revert
   the mutation.yml src wiring; mutation coverage for src-authored modules is a
   separate follow-up tracked in #537. (The deletion of the gitignored top-level
   .cjs does not match the workflow's `bin/lib/**/*.cjs` git pathspec, so the
   gate skips cleanly.)

Verified: clean-room `npm ci --dry-run` exits 0; deleting the artifact then
running a suite rebuilds it; run-tests harness 22/22 green; `npm pack` includes
the built artifact via prepack.

package.json, so their expected literals must follow the rename. The drift-lint
unit test is left as-is — its SEAM is a self-consistent fixture and its
stale-literal detection cases would shift if altered; the live-repo scan in it
already passes.

6f2520786d feat(#498): single Package Identity seam for /gsd:update + fix runtime undefined-name bug (#499)
* feat(#498): generated package-identity seam derived from package.json

Introduce a single source for GSD's published-package coordinates:
scripts/generate-package-identity.cjs (pure deriveIdentity + formatManualInstall
+ render) emits the generated get-shit-done/bin/lib/package-identity.cjs with
values baked from package.json at build time. Baking is required because the
installed tree carries only a synthetic {"type":"commonjs"} package.json, so a
runtime require('package.json').name resolves to undefined (#378). Reconciles

Wired into npm run build; a parity test fails CI if the committed file drifts
from package.json.

scripts/lint-package-identity-drift.cjs scans the runtime/code surface
(bin/, hooks/, scripts/, get-shit-done/) and asserts every GSD package name
and GitHub repo slug literal equals the Package Identity seam's current value.
Passes today; fails the moment a repoint isn't propagated (rename package.json,
regenerate the seam, and stale literals are reported until updated). This is
the second adapter that makes the seam real and a repoint mechanically safe.

Enforced via tests/issue-498-identity-drift-lint.test.cjs (scanRepo === [])
under npm test; also exposed as `npm run check:identity-drift`.

update.md's ~280-line get_installed_version bash. resolveUpdateContext()

global probe via env overrides then $HOME -> LOCAL-if-distinct -> scope
cascade -> UNKNOWN) and returns the 4-field contract { installedVersion,
scope, runtime, gsdDir }. The fs is injected so every branch is finally
testable without a live multi-runtime install.

Expose it as `gsd-tools update-context [--config-dir <d>] [--runtime <r>] --json`.
Purely additive — update.md is unchanged in this commit; the workflow swap
follows separately.

same cascade — but the logic is now tested in update-context.cjs instead of
untestable bash-in-markdown.

Relocate the #3608 antigravity-first-class contract onto the projection
(RUNTIME_DIRS order, inferPreferredRuntime, envRuntimeDirs) plus a behavioral
test; keep the execution_context path-classification assertion on update.md.
Re-point install.test's custom-config-dir assertion (kilo.jsonc/KILO_CONFIG)
to update-context.cjs where that detection now lives.

Full root suite: 2022 pass / 0 fail.

  which Windows git checks out as CRLF (no .gitattributes eol rule). Normalize

was skipped — and since the update then runs a clean install that wipes managed

UNKNOWN scope, which still skips the backup). Add a structural regression

De-hardcode the repo slug + install command in the changeset tooling — #516
only single-sourced the package NAME, leaving 'open-gsd/get-shit-done-redux'
hardcoded in scripts/changeset/cli.cjs and github-release-notes.cjs. Route both
through the seam's repoSlug/packageName so a rename is a regenerate, not a hand
edit. The drift-lint real scan now reports zero divergent coordinate literals.

Projection parity vs the old inline bash, as ONE consistent rule
(trustedVersionAt) applied on every path:
 - expand a leading ~/ in preferredConfigDir before the fast path (the bash ran
   expand_home first; a custom --config-dir ~/foo otherwise fell to UNKNOWN)
 - trust a version only when BOTH VERSION and the update.md marker exist — fast
   path AND LOCAL/GLOBAL cascade; a partial dir falls to 0.0.0 keeping scope
 - apply the same same-path dedup to the 0.0.0 fallback so a partial install
   probed from cwd===home is not misdetected as LOCAL

Adds regression tests for tilde expansion, VERSION-only (cascade + fast path),
and the cwd===home partial-install dedup.

Adds get-shit-done/bin/lib/package-identity.cjs as the single source of
truth for PACKAGE_NAME, derived from package.json `name` via require.
Refactors all runtime code-line occurrences in bin/install.js,
get-shit-done/bin/check-latest-version.cjs,
get-shit-done/bin/lib/shell-command-projection.cjs,
get-shit-done/bin/lib/verify.cjs, scripts/changeset/cli.cjs,
scripts/changeset/github-release-notes.cjs, and
scripts/release-tarball-smoke.cjs to import PACKAGE_NAME from the
identity module instead of hardcoding the literal.

The package name is unchanged (@opengsd/get-shit-done-redux). Behaviour
is byte-identical: all --help, hint, and release-notes strings render
exactly as before. Golden-literal tests (bug-2992, bug-378) keep their
hardcoded expected values and remain GREEN.

Adds tests/package-name-single-source.test.cjs lint guard: fails CI if
@opengsd/get-shit-done-redux appears as a code-line literal in runtime
.cjs/.js outside the identity module, enforcing a one-file rename path.

a7ed001b27 fix(#494): ci-test-scope selects checks a diff can break (tests->full matrix, docs->docs-parity) (#495)
classify() under-approximated breakable checks, so scoped PRs skipped the
check their diff would break and regressions reached next (#484 docs-parity,
#482 windows-22 EBUSY). Fail-safe widen: any tests/** change forces
full_matrix (OS-specific test failures); any docs/**, commands/**, agents/**
change marks code_changed and selects docs-parity-live-registry (its runtime
inputs). Updated the docs-only test that asserted the old buggy contract.

d3eaf6aec1 docs(#23): document changeset extract CLI contract (#479)
Add scripts/changeset/README.md specifying the cli.cjs extract
subcommand: invocation, flags, version validation, exit-code table
(0/1/2), and output shapes for text and --json modes.

Corrects two inaccuracies from the triage table against the source:
v-prefixed versions ARE accepted (stripped), and it is pre-release/
build suffixes that are rejected — not the v prefix. Also documents
the full exit-1 surface (missing flags, invalid semver, missing
changelog) and the exit-2 overlap (empty range vs malformed argv) so
external callers do not conflate "no releases in range" with failure.

Closes #23

The branch commit 4e630d99 stripped `allow-test-rule: pending-migration-to-typed-ir`
from ~80 test files without replacing their assertions or adding the correct
exemption annotation. The files were NOT source-grep tests — they read .md
workflow/agent/command/reference files (source-text-is-the-product) or hook
source files for structural invariants (structural-regression-guard). No
assertion logic was changed; only the correct allow-test-rule annotation was

Per actions/runner workflow-v1.0.json schema, `jobs.<job_id>.defaults.run.shell`
allows `matrix` context (job-defaults-run has context:[matrix,...]); step-level
`shell:` does not (run-step's shell field is plain string with no context array).
PR #434 used step-level shell:${{matrix.shell}}, which GHA's parser rejects with
"Unrecognized named-value: 'matrix'" — blocking every push to next and every
release.yml dispatch.

This commit:
- Removes step-level `shell: ${{ matrix.shell }}` from test-full (test.yml)
  and smoke (install-smoke.yml) jobs (17 directives).
- Adds `defaults.run.shell: ${{ matrix.shell }}` at job level in those two jobs.
- Fixes pre-existing shellcheck SC2129 in test.yml (individual >> redirects →
  grouped brace form) and SC2010 in install-smoke.yml (ls|grep → glob loop).

Verified locally with actionlint 1.7.12 (exit 0). Policy linter still 0 violations
(matrix.shell now resolves via job.defaults.run.shell which the linter already
handles per workflow-policy.cjs:effectiveShell).

is only activated on Windows; macOS/Linux continue to resolve the plain
npm binary directly with shell: false, preserving the existing behaviour
on non-Windows platforms.

Updated both spawnSync(npmCmd, ...) call sites:
- npm --version check (line 182)
- npm ci --dry-run lockfile-sync check (line 215)

On Windows, os.homedir() reads USERPROFILE (not HOME), so the test's

pre-commit test, the real git output nothing (no staged files on a fresh
checkout), so the grep match failed and npm was never called.

Fix: set MSYS2_PATH_TYPE=inherit in the env passed to both bash spawns.
With inherit, MSYS2 uses only the converted Windows PATH without prepending
system directories, so binDir (converted from Windows to POSIX) is first in
the search path and the mock stubs are found.

grep/tr/printf remain available: the GHA Windows runner PATH includes
C:\Program Files\Git\usr\bin which contains these utilities; MSYS2 converts
that Windows entry to a POSIX path on startup. The /usr/bin/env shebang in
mock stubs resolves through MSYS2's virtual filesystem mount (not via PATH)
and is always accessible regardless of MSYS2_PATH_TYPE.

On macOS/Linux this variable is ignored; no behaviour change on those platforms.

The CI failure showed a test duration of 120003.1812 ms — matching the
previous CHILD_TIMEOUT_MS = 120_000 exactly. When spawnSync hits its
timeout, it sends SIGTERM and returns { status: null, stdout: '', stderr: '' }
per the Node.js docs:

Also expose `signal` and `installError` in the INSTALL_FAILED details object
so a future timeout (status=null, signal='SIGTERM', stdout='') is immediately
diagnosable in CI logs without guesswork.

* fix(#437): chmod +x via bash on Windows for hook test mocks (root cause: fs.writeFileSync mode=0o755 no-op on NTFS)

Root cause: Node's fs.writeFileSync mode=0o755 is a no-op for the execute
bit on Windows NTFS. Per https://nodejs.org/docs/latest-v22.x/api/fs.html:
"on Windows only the write permission can be changed." Bash's access(X_OK)
therefore skips the mock file; the real git/npm binary is found later in PATH
and the hook runs against real state instead of the test double.

Fix: after writeFileSync, invoke Git Bash's chmod via the POSIX emulation
layer (Cygwin/MSYS2), which sets the NTFS execute ACL that Node cannot reach:

execFileSync('bash', ...) works because Git for Windows ships bash on PATH in
all GHA Windows runners. Forward-slash conversion is required because MSYS2
bash auto-converts /c/foo paths but not mixed-separator paths.

Why prior approaches didn't take effect:
- MSYS2_PATH_TYPE=inherit: only read in /etc/profile (login-shell path);
  execFileSync('bash', ...) launches non-interactively without --login, so
  /etc/profile is never sourced.

  not honor PATHEXT, so wrappers are not found by bash's own PATH scan.

Four prior rounds (path.delimiter join, MSYS2_PATH_TYPE=inherit, cmd-shim
.cmd/.ps1 wrappers, chmod-via-bash post-write) all failed to make MSYS2
bash's PATH-lookup find the mock executables. The root cause is that none
of those approaches can reliably override bash's own command-resolution
on NTFS without fighting NTFS execute-ACLs or login-shell profile sourcing.

The simplest robust solution is to bypass PATH entirely:

Hooks: each hook now binds GIT_CMD="${GIT_OVERRIDE:-git}" (and NPM_CMD for
pre-commit) at the top. When env vars are unset the hooks invoke bare
`git`/`npm` exactly as before — zero behavior change for users.

Tests: writeMockBin/binDir/PATH manipulation replaced by writeMock(), which
writes a .sh mock to a tmpDir and passes its absolute path via GIT_OVERRIDE
/ NPM_OVERRIDE in the execFileSync env. Bash inside the hook executes the
path directly via the seam — no PATH scan, no NTFS ACL check, no MSYS2
profile dependency.

Test-rigor principle: the new seam (env-var injection) is platform-
independent and doesn't rely on bash's command-resolution mechanism on
the host OS.

Adds tests/policy-shell-pinning.test.cjs: 8 tests (baseline + 6 synthetic
counter-tests). Synthetic tests 2–7 pass; baseline test is intentionally RED
(37 violations: 28 in test.yml, 9 in install-smoke.yml — all macos/windows
lanes using shell: bash instead of native zsh/pwsh).

Adds js-yaml@4.1.1 as devDependency for YAML parsing.

* fix(#431): switch ubuntu/windows lanes to native shells; extract bash-isms to Node

Remove all explicit shell: bash pins from ubuntu-only jobs (changes, lint-tests,
coverage, required-tests, smoke-unpacked) — ubuntu runner default is bash, which
is both H1-compliant and the runner default, making the pin redundant.

For the test and test-full mixed-OS jobs (ubuntu+windows, windows+macos):

The old test required all Windows-targeting npm steps to pin shell: bash
(to prevent pwsh stderr-swallow). Under H1, Windows runners must use
pwsh (native, no pin needed) — shell: bash on Windows is now the

Update findViolations() to flag npm steps with effectiveShell === 'bash'
(rather than effectiveShell === null). Update synthetic tests to verify

8c8887f00e fix(#370): scope affected-tests runner to PR suites, exclude push-only install/slow (#395)
Root cause: the affected-tests runner called runAllSuites() on critical-path
changes (running every suite including install/slow on all matrix cells including
Windows), and pickAffectedTests injected DEFAULT_SMOKE_TESTS (an install test)
as the empty-selection fallback — causing install suite tests to run on PR lanes
where they are push-only per docs/TESTING-SUITES.md.

Fix: PR_EXCLUDED_SUITES filter at the pickAffectedTests chokepoint strips
install/slow from every selection path (direct-change, reverse-index, stem-match).
Empty selection now returns [] and the caller runs the unit suite as smoke.
Critical-path fallback replaces runAllSuites with PR_FULL_SUITES
(unit, integration, security). suiteOf exported from run-tests.cjs
(with require.main guard) so affected-tests-lib reuses canonical detection.

Workflow bash blocks resolved the runtime as GSD_SDK="node $GSD_TOOLS" and
invoked it unquoted ($GSD_SDK query ...). On install paths containing spaces
(e.g. /Volumes/Mini Me/...) the unquoted expansion word-split into
`node /Volumes/Mini gsd-tools.cjs ...`, failing with "Cannot find module
'/Volumes/Mini'" and getting masked by `2>/dev/null || echo "{}"` into a
silent empty state.

Replace the string variable with a single-line shell launcher that defines a
gsd_run function, invokes the runtime with a fully-quoted path and "$@", and
preserves the local-cjs / installed-gsd-tools-on-PATH fallback (#3668) plus
the loud not-found error and install hint. The launcher uses _GSD_SHIM_NAME
indirection so no workflow emits the /gsd-tools substring that the do.md
dispatcher-parity scanner would misread, and is single-line to stay within
the per-file progressive-disclosure line budgets (#2551).

The canonical launcher lives in
get-shit-done/workflows/_runtime-launcher.snippet.sh, is propagated once per
file by scripts/sync-runtime-launcher.cjs, and is locked by
tests/runtime-launcher-parity.test.cjs (fails CI on drift, on a reappearing
$GSD_SDK token, or on a /gsd-tools substring). Dependent workflow-assertion
tests are updated from $GSD_SDK to gsd_run, and the runtime launcher is
registered in CONTEXT.md.

3169d5cda6 fix(ci): reduce Windows test concurrency 4→2 to prevent synckit worker exhaustion on Node 24 (#173)
Under Node 24 on Windows, running node --test with --test-concurrency=4
causes 4 concurrent gsd-tools subprocesses to each spawn a synckit
worker_threads worker for the SDK bridge. The 4 workers simultaneously
contend on SharedArrayBuffer + Atomics.wait under Windows Defender
scanning and NTFS latency, triggering OS-level resource exhaustion that
kills worker processes with empty stderr before any output is flushed.

The symptom: intermittent exit 1 with 0 test failures, varying affected
test files per run, all sharing the pattern of invoking gsd-tools as a
subprocess. Empty stderr distinguishes OS crash from gsd-tools app error
(the error() path writes to stderr before exiting).

Fix: platform-aware concurrency default — 2 on win32, 4 on Linux/macOS.
The existing TEST_CONCURRENCY env-var override is preserved. Also adds
a [stderr: (empty) exit:N] diagnostic note in helpers.cjs runGsdTools
catch block so future empty-stderr crashes are visible in CI logs.

I/O adapter pattern: pure logic shared; each side (CJS sync, SDK async) supplies
its own I/O adapter.

The pure functions are defined in the generator as real JS functions and serialized
via Function.prototype.toString() — same technique as gen-project-root.mjs — rather
than embedded in template literals (which would require double-escaping all regex
backslashes).

   inside the string, causing require() to fail silently (2>/dev/null
   || true). engines.node returns empty, triggering a spurious FAIL.
   Fix: switch to fs.readFileSync + JSON.parse and normalise
   backslashes to forward-slashes before embedding in the JS literal.

Also pass { CI: '' } from the bad-nvmrc unit test so the
version-manager-pin fixture test still exercises the mismatch path
even when running inside CI runners.

7bd77d6268 fix(116): locale-safe base64-scan with portable timeout and partial-scan signaling (#132)
* test(116): reproduce base64-scan illegal byte sequence on non-UTF8 fixtures

Adds regression fixtures and failing tests for #116. Empirically verified
on macOS 26.5 (BSD tr) that `tr -cd '[:print:]'` under LC_CTYPE=en_US.UTF-8
exits non-zero with "Illegal byte sequence" when its input contains bytes
that are not valid UTF-8 start sequences (e.g. lone continuation bytes 0x80–0x9F).

The base64-scan.sh root cause is a known bash pitfall: the assignment
  `local printable_count=$(... | tr -cd '[:print:]' | ...)`
uses `local` on the same line, which always returns exit 0, masking the tr
failure. Result: tr errors surface only on stderr; the scan exits 0 with
incomplete coverage (false-clean signal).

Two new tests FAIL on origin/main:
  - "scans non-UTF8 file containing a b64 blob without emitting Illegal byte sequence"
  - "dir scan with non-UTF8 files under non-C locale completes cleanly within 30s"

Fixtures in tests/fixtures/base64-locale/:
  utf8-with-injection.md       — UTF-8 + base64-encoded injection (positive control)
  non-utf8-with-b64blob.bin    — raw 0x80-0x9F bytes + b64 blob that decodes to
                                 binary (this is the reproducer that triggers tr error)
  mixed-encoding.txt           — valid UTF-8 + lone continuation bytes
  clean-text.md                — negative control (must not be flagged)

Test helpers use spawnSync (not execFileSync) so stderr is captured even on
exit 0 — execFileSync only surfaces stderr via the thrown error on non-zero exit.

start sequences with "Illegal byte sequence" when LC_CTYPE is set to any
UTF-8 locale (e.g. en_US.UTF-8). Empirically verified on macOS 26.5 using
`man tr` (ENVIRONMENT section) and direct testing:
  printf '\x80\x81hello' | LC_ALL=en_US.UTF-8 tr -cd '[:print:]'
  → tr: Illegal byte sequence (exit 1)

The error is silently masked because base64-scan.sh uses `local` on the same
line as the tr assignment. Bash's `local` built-in always returns 0 regardless
of the subshell's exit code — so the tr failure never propagates under
`set -euo pipefail`. Result: the script exits 0 with truncated printable_count,
causing binary-decoded blobs to be skipped (false-clean, security gap).

Fix: `export LC_ALL=C` at script level (line 33).
  - LC_ALL=C forces the POSIX C locale throughout: tr treats every byte 0x00–0xFF
    as a valid character, never rejects high bytes.
  - Safe for all script operations: all injection patterns are ASCII, grep POSIX
    classes ([:space:], [:print:]) behave correctly in C locale, base64 -d is
    locale-independent.
  - Script-level export is appropriate because all operations in this script are
    byte-level; no multi-byte character handling is needed.

Additional hardening:
  - MAX_LINE_BYTES=1048576 guard in extract_and_check_blobs: lines longer than
    1 MiB are skipped with an explicit "partial scan" warning to stderr. This
    bounds grep -oE cost on pathological inputs (minified JS, single-line binary
    blobs) and satisfies the "partial-scan failure signaling" requirement.
  - Portable run_with_timeout + is_timeout_exit: probes for GNU timeout,
    gtimeout (homebrew), and falls back to perl alarm(N)+exec. Defined for
    future use guarding external sub-commands. Verified: no timeout binary on
    this macOS host, perl alarm fallback works correctly (exit 142 on SIGALRM).

Test-rigor fixes applied per test-rigor skill review:
  - Fixture validity check: assert `isInvalidUtf8` (round-trip length difference)
    rather than checking for a specific byte range — the property that matters is
    "file is not valid UTF-8", not "file has bytes in 0x80–0x9F".
  - FAIL assertions: assert `FAIL: ${INJECTION_FIXTURE}` (specific filepath) not
    `result.stdout.includes('FAIL')` — rules out false-positives on other fixtures.
  - Test name: renamed "mixed-encoding file does not cause scan to abort or hang"
    to accurately describe what is tested (no extractable blobs → exits clean).

npm reads $HOME/.npmrc (user config) and writes to $HOME/.npm (default
cache dir) unless overridden. On Docker hosts the running user's HOME
may be uninitialized, unwritable, or contain stale state from prior
runs — any of which causes `npm pack` / `npm install -g` in the
before() hook to fail with EACCES, cancelling all 6 subtests (A–F).

Fix: allocate a fresh mkdtemp dir once per test process in helpers.cjs
and inject it as HOME, npm_config_cache, and npm_config_userconfig for
every runNpm() call. A process.on('exit') handler removes the dir on
teardown. The caller-supplied env option (if any) is merged on top of
the isolated env so explicit overrides still win.

- Test 1: runNpm succeeds when process HOME is chmod-0500 (unwritable)

* fix(3496): wire extract into update.md + reject pre-release in range, fix CHANGELOG parser edge cases

BLOCKER fixes:
- F1: workflows/update.md show_changes_and_confirm step now invokes
  `scripts/changeset/cli.cjs extract --from $INSTALLED_VERSION --to
  $LATEST_VERSION --changelog $CHANGELOG_TMP --json` with explicit exit-2
  handling ("no releases in range") and fallback text.  The prior prose
  ("extract entries between versions") was never wired to the binary and
  silently skipped intermediate versions (#3496).
- F2: releases.filter in cmdExtract now rejects any rel.version that does
  not pass SEMVER_RE before numeric-tuple comparison.  parseSemver('1.0.0-rc.1')
  previously returned [1,0,0] (same as '1.0.0'), causing pre-release entries to
  corrupt range queries.  Architectural choice: skip pre-release + 4-part
  versions with a stderr warning; full semver §11 pre-release ordering deferred
  to a consolidation issue (see F8 note below).

MAJOR fixes:
- F3 (serialize.cjs): releaseMatch regex updated to
  /^##\s+\[([^\]]+)\](?:\([^)]*\))?\s*(?:-\s*(\S+))?/ so linked-header
  format `## [1.42.1](url) - 2026-05-15` captures the date correctly.
- F4 (serialize.cjs): continuation-line test now checks `!/^\s+-\s/`
  so `  - nested item` terminates the current bullet instead of folding in.
- F5 (cli.cjs): 4-part versions (e.g. 1.0.0.1) fail SEMVER_RE and are
  skipped by the same guard added for F2.  No separate code path needed.
- F6 (serialize.cjs): v-prefix stripped from in-file version capture;
  `## [v1.0.0]` now parses as version "1.0.0".
- F7 (serialize.cjs): continuation-line indentation relaxed from /^[ \t]{2}/
  to /^\s+/ so 1-space-indented continuations fold correctly (F4's
  bullet-terminator guard prevents nested bullets from being folded).

MINOR fixes:
- F9 (cli.cjs): unknown-command path now exits 1 instead of 2 (exit 2
  is reserved for "no releases in range" semantic).
- F10 (cli.cjs): text-mode exit-2 path now writes
  "no releases found in range (from=X, to=Y)" to stderr.
- F11 (tests): exit-2 test now asserts r.json is present and
  r.json.releases.length === 0.

NOTE — F8 (semver consolidation): this codebase has 5+ distinct semver
comparators with divergent pre-release policies; this PR adds a 6th (the
SEMVER_RE guard in cmdExtract).  A follow-up consolidation issue should be
filed to unify all call sites.  Out of scope for this PR.

Regression tests added for F1–F6: pre-release exclusion, linked-header
date parsing, nested-bullet termination, 4-part/v-prefix edge cases, and
workflow wiring.  All 15 tests pass.

Checks in JSON specs for three rulesets (main-protection,
release-branches, tag-immutability), a CODEOWNERS file (advisory),
and scripts/sync-rulesets.sh to apply them.

Enforcement is `disabled` in all three files — PR-2 will apply with
`evaluate` for a 1-week dry-run, PR-3 will flip to `active`.

  false-failing
- When no pristine_hashes entry exists (older installer / absent backup-meta),
  fall through to the pre-fix behaviour (use on-disk pristine as-is)
- Export sha256, readPristineHashes, and OK_PRISTINE_DRIFT_DETECTED

New REASON code OK_PRISTINE_DRIFT_DETECTED is added to the frozen enum.
Exit code contract is unchanged: 0 for gate pass (including skipped-due-drift
files), 1 for real user-content failures, 2 for structural errors.

The fix is to raise the ceiling from 9 to 10 and set issue=3767.
This file does not exist on this branch yet (introduced on main after
the branch diverged) so we add it here. The merged CI state will see
the bumped ceiling and pass.
