6b0b92674a refactor: remove dead descriptor-driven mechanisms (no remaining consumer)
Drop code whose only consumer was a retired runtime: hostBehaviors readers for
agentFileExtension, localTargetIsProjectRoot, sharedHooksDirName,
skillsManifestPrefix and skipCodexSkillsManifest; the empty
NON_REGISTRY_CONFIG_HOME_DESCRIPTORS array and live-config-guard plumbing; the
empty RUNTIME_NOTE_AUDIENCE_BY_HEADING filter; the unused resolveVersionFrom
export; and the WINDSURF_SESSION_ID workstream session key. Delete tests that
only exercised those mechanisms.

a9a7a328e6 refactor: hard-fork GSD -> MSD (Make Software Done)
Mechanical rename produced by scripts/msd-rename.cjs: gsd/Gsd/GSD -> msd/Msd/MSD
across contents and paths, upstream package/repo coordinates -> @golem15/msd-core
and golem15com/msd-core. Deep links into upstream history, sibling upstream
packages, the GSD-2 import feature, CHANGELOG.md and .changeset/ are kept as-is.

Hand edits on top: MSD block-letter banner and logos, LICENSE copyright line,
package/plugin identity, regenerated lockfile, install-tree fixtures, derived
registries and benchmark baseline; migration checksum baseline re-locked
(MSD keeps its own install state, so no install had applied the old sums);
sort-order and regex-escaped expectations in tests adjusted.

new-project.md's Step 5.1 (Sub-Repo Detection) ran its own bash predicate:

  find . -maxdepth 1 -type d -not -name ".*" -not -name "node_modules" \
    -exec test -d "{}/.git" \; -print

`test -d` requires .git to be a DIRECTORY. A linked git worktree's .git is
a FILE (a `gitdir: <path>` pointer), so this predicate silently excluded
valid linked-worktree children while still finding ordinary clones.

src/core-utils.cts's detectSubRepos(cwd) already handles this correctly
(fs.existsSync, type-agnostic) but had zero callers anywhere in the
codebase -- orphaned logic the workflow never actually used, despite
duplicating a narrower version of the same check inline.

Wired detectSubRepos into cmdInitNewProject's JSON output as a new
sub_repos_detected field (matching the file's existing pattern of
similar directory-scan-derived fields like has_existing_code/
is_brownfield/has_codebase_map) and replaced the workflow's raw find
fence with a gsd_run query init.new-project call reading that field --
removing the duplicate, narrower detection logic entirely rather than
patching it in place, per the issue's own "reuse a central policy"
framing.

Added the missing .git-as-FILE test case to the existing
tests/core-utils.test.cjs detectSubRepos coverage (proving the helper
was already correct -- the defect was entirely in the unwired workflow
predicate) plus CLI-level end-to-end coverage in
tests/init-manager.test.cjs using a REAL `git worktree add` fixture,
matching the issue's own reproduction steps, alongside an ordinary
child-clone case and a non-repository-directory negative case.

Refreshed tests/fixtures/compact-content-benchmark-baseline.json (gsd-test
caught the drift from new-project.md's byte-count change; the benchmark
script itself always exits 0 -- report, not gate -- but the wrapper test
enforces the committed baseline stays in sync).

Emitted-Drift-Ack-Growth: new-project.md — #4458 replaces the raw find predicate in Step 5.1 with a gsd_run query call reading the new sub_repos_detected field, net +140 bytes

CI's lint-tests caught two ESLint findings my local gsd-test run
couldn't see (gsd-test's matrix doesn't run npm run lint:ci -- same gap
already observed on #4456's PR):

- local/no-unbounded-spawn: the new execFileSync('git', ['worktree',
  'add', ...]) call had no timeout, an indefinite-hang risk.
- local/no-adhoc-timeout-literal: my first fix (a bare `timeout: 15_000`
  literal) was itself flagged -- two independent hardcoded copies of a
  guessed timeout can silently drift or collide (this repo hit exactly
  that on 2026-09-06, PR #4428).

Fixed by importing GIT_FIXTURE_TIMEOUT_MS from tests/helpers/timeouts.cjs
-- `git worktree add` checks out files into a new working tree, the same
"construction" weight class as init/config/add/commit that constant
already covers, not plain plumbing (GIT_TIMEOUT_MS's class).

Verified via `npm run lint` directly (clean) before re-running gsd-test.

* fix(#4458): reduce redundant git subprocess overhead in new tests

CI's full-test Windows shard 2/3 failed: chunk 1/9 (306 files) exceeded
its internal 600s budget and was force-killed, with an unrelated file
(codex-config.test.cjs) in flight at the moment of the kill -- meaning
the chunk's AGGREGATE runtime, not any single hang, blew the budget.

This PR's own three new tests each independently called
createTempGitProject() (git init + a commit), and one of them also runs
git worktree add -- real subprocess spawns, each Defender-scanned on
Windows CI (tests/helpers/timeouts.cjs's own documented rationale for
why Windows spawn classes get generous budgets). That's a genuine,
quantifiable overhead addition to the exact chunk that timed out, not
something to wave off as unrelated flake without checking.

Two of the three tests never actually needed a real git repo --
detectSubRepos only inspects a CHILD directory's own .git, never the
root's git state, and the existing SUBCOMMANDS loop earlier in this same
file already proves `init new-project` succeeds against a plain,
non-git createTempProject() fixture. Switched those two tests to the
lighter fixture, leaving only the one test that genuinely needs a real
repo (git worktree add requires one) on createTempGitProject().

9ee6d54cc3 fix(#4306): extend fault-injection fd-swallow fix across the whole suite (#4308)
* fix(#4306): forward real bytes through io.test.cjs's fault-injection mocks

The bug #1008 fault-injection tests mock fs.writeSync scoped only by file
descriptor. On their "success" arms (the retry-after-EAGAIN/EINTR call, and
the short-write simulation) they fabricated a return byte count without ever
calling the real writeSync -- the bytes went into a local array and nowhere
else.

node:test's process-isolation runner (default on Node >= 22) reads each test
file's own stdout to parse its child-to-parent result protocol. If the
runner's own reporter write for an adjacent test lands on fd 1 while one of
these mocks is installed, that write was silently swallowed instead of
reaching the real pipe -- observed in CI as "Unable to deserialize cloned
data" (a corrupted/truncated byte stream on the parent's read side), not a
thrown exception.

Every "success" arm now forwards the real bytes to orig()/restore() instead
of fabricating a return value, so anything else sharing the fd during the
mocked window still gets its bytes delivered for real. writeAllSync (the
only production caller reaching this mock) always passes a Buffer, so the
forwarded calls use the buffer-form fs.writeSync overload unambiguously.

The originally-fixed instance (tests/io.test.cjs) was one occurrence of a
copy-pasted defect: mocked fs.writeSync arms fabricated a return byte count
without ever forwarding the call to the real fs.writeSync, silently
discarding bytes. Under node:test's process-isolated runner, the parent
reads the child's real stdout to parse v8-serialized report frames
interleaved with plain output (confirmed against node's own
lib/internal/test_runner/runner.js and a matching upstream issue,
nodejs/node#64061) — a swallowed write on that fd corrupts the parent's
parse ("Unable to deserialize cloned data").

Adds a shared, safe capture helper to tests/helpers.cjs, captureFdSync(fd,
fn): it always forwards every write to the real fs.writeSync first, then
records only the observed fd's bytes, sliced by the real return count (not
the requested length), decoded once via Buffer.concat so a short write
can't split a multi-byte codepoint across two decodes.

17 test files migrate their local copy of the unsafe mock to this shared
helper. tests/worktree-base-ref.test.cjs keeps a narrower in-place fix
instead (it needs to record every fd a write touched, which the shared
helper doesn't expose).

tests/io.test.cjs gets two follow-up correctness fixes on top of the
already-committed forwarding fix: the EAGAIN/EINTR/short-write arms now
derive their recorded chunk from the real return count everywhere
(including the string-form overload), and the short-write test no longer
forces a Buffer-shaped truncation call onto a string-form write that could
land on the same fd.

389bc86e02 enhance(#3883): route every slug call site through its canonical owner (#3896)

I wrote this section in Phase 0 stating rules I had not executed against the
tree. Measured at 832dcbb75, three statements are wrong.

The slug count is 11 across 5 files, not 13; two of the thirteen were an
unrelated tokenizer regex. The divergence is real and reproduced.

resolveRuntime reads no install marker at all and has no cache; PR #3382,
cited as prior art for that rung, is closed unmerged.

The Codex sandbox is still a hand-maintained subset map with a silent
read-only fallback, and validate agents checks file presence only -- both
halves of that claim are false.

The shortFormToId rule is accurate. The guard roster names no casualty for
this rule.

Section 8.3 is therefore a work list, not a conformance check.

PR #3896's Stryker (core-utils) shard was cancelled at the 15-minute
shard cap. Stryker's command-runner bills one `node --test <file>`
invocation as a single unit costing whatever the file's slowest run
costs, re-run once per mutant (documented in
tests/state-contract.test.cjs's header, #2790 precedent). A3/A5 drove
every CLI-reachable slug site through runGsdTools (a real child-process
spawn per call, ~85-170ms each across ~70 calls), accounting for ~7.1s
of the file's ~7.9s wall time.

commands.cts:cmdGenerateSlug and init.cts:cmdInitExecutePhase/
cmdInitPhaseOp/cmdInitProgress are plain functions reachable in-process
from the built gsd-core/bin/lib/*.cjs, so this calls them directly
instead of spawning gsd-tools, capturing their fd-1 JSON output with the
bug #1008 fs.writeSync-mock pattern already used in tests/io.test.cjs
and tests/init.test.cjs. A hermetic-env helper reproduces the isolation
runGsdTools's { HOME: tmpDir } + testEnvBase() gave the child process.

File wall time drops from ~7.97s to ~1.1s (246/246 passing, same
coverage), well inside the 15-minute cap even at hundreds of mutants.

extractOneLinerFromBody matched the first heading's first bold run regardless

343835facc refactor(#3183): route live-plan counting through scanPhasePlans (#3199)
* refactor(#3183): route live-plan counting through scanPhasePlans

scanPhasePlans becomes the sole owner of the live-plan derivation. Twenty-one
independent re-derivations across seven modules now route through it, and
scripts/lint-plan-count-drift.cjs reports zero, scanning the whole repo rather
than an allowlist (ADR-3180 Decision 4a).

The epic scoped this at three copies. A whole-repo guard found twenty-six sites
across nine files, so Phase 1 absorbs every live-plan re-derivation and Phase 3
narrows to window plus sentinel enumeration.

Two sites are exempt with a documented reason rather than a bare allowlist:
audit.cts scans one quick task's own directory for a single completion record,
and gsd2-import.cts reads a foreign GSD-2 tasks/ layout during a one-time
import. Neither is a phase directory.

scanPhasePlans gains allPlanFiles (pre-supersession) alongside planFiles so one
owner answers both questions: verify.cts's numbering-gap check wants every plan
on disk, its pairing check wants the live set. Both fields are additive.

Highest-severity fix: cmdPhasePlanIndex, which feeds execute-phase wave
scheduling, was scheduling status:superseded plans into waves and reporting zero
plans for the post-#3139 nested layout.

filterPlanFiles and filterSummaryFiles are deleted; getPhaseFileStats orphaned
them and only their own tests still called them.

New leaf module src/planning-scope.cts carries the frozen SCOPE discriminator,
with its six-gate ripple closed: gitignore, inventory manifest, INVENTORY.md and
the CONTEXT.md glossary.

Records the amended scope, scanPhasePlans's new allPlanFiles field,
findOrphanSummaries, the two documented exemptions, the re-derived Tier-2
table, and the describeNonCanonicalPlans trap for later phases.

The remote runner went red with 13 deterministic failures on both lanes,
and they were right: replacing verify.cts's canonicalPlanStem pairing with
summaryCandidates dropped a case the bespoke rule covered. A plan carrying a
descriptive slug after its id (68-01-scaffolding-PLAN.md) pairs with its
canonical-stem summary (68-01-SUMMARY.md), and summaryCandidates generated no
such candidate, so the plan read unsummarized.

The fix is to complete the one rule rather than restore a second:
summaryCandidates gains a canonical-id candidate, narrowed to fire only when an
id pair was actually extracted. countMatchedSummaries, findUnsummarizedPlans
and findOrphanSummaries all inherit it. The two-plans-one-summary collision
behaviour of the original rule is preserved deliberately and documented in
place.

Second defect, independently root-caused while verifying: routing the #2893
naming diagnostic through scanPhasePlans exposed it to the loose /PLAN/i
fallback, which is correct for counting and wrong for a naming check — a
non-canonically-named file was accepted as a valid plan and the diagnostic
went silent. cmdPhasesList, cmdFindPhase and cmdPhasePlanIndex now intersect
with a strict isCanonicalPlanFile predicate before reporting names.

Same class as the describeNonCanonicalPlans trap already recorded in ADR-3180:
a question about file naming wants the physical, strictly-matched set; only a
question about outstanding work wants the live set.

tests/repo-invariants.test.cjs asserts every bin/lib/*.cjs is linted xor
ignored per its ADR-457 migration state. The new planning-scope module closed
five of the six .cts ripple gates - gitignore, inventory manifest, INVENTORY.md
and the CONTEXT.md glossary - but not eslint, because that one is enforced by a
test rather than by lint:ci, so the local pipeline stayed green while it was
missing.

Generated from src/planning-scope.cts, so the .cjs is ignored and the .cts is
linted, matching every other migrated module.

* fix(#3183): replace the plan-count drift detector with a literal tokenizer

CodeQL reported 4 high-severity js/redos alerts on REGEX_LITERAL_MD_RE, the
backtracking regex that finds "a regex literal mentioning PLAN/SUMMARY and an
escaped \.md". Five review rounds found it had two defects, not one:

  - EXPONENTIAL, then CUBIC. Its "any char" atom `(?:\\.|[^/\r\n])` let a `\.`
    pair be consumed either as one escape or as two class characters, which is
    exponential backtracking: 27,464ms on `"/\.mdplan" + "\.".repeat(28) + "X"`.
    Excluding `\` from the class killed that but left a cubic path — 23ms at
    N=200, 172ms at N=400, 1362ms at N=800 on `"/" + "PLAN\.md".repeat(N)` with
    no closing `/`. This guard is the last stage of `npm run lint:ci`, which CI
    runs on fork pull requests, so a crafted src/*.cts could stall the job.
  - A DETECTION HOLE. A character class holding a bare, unescaped `/` — e.g.
    `/SUMMARY[^/]*\.md$/`, an ordinary path-excluding filter — terminated the
    literal at that `/`, so the scan never reached `\.md` and the guard missed
    it entirely. (Classes holding an ESCAPED `\/` were already matched; the
    tests cover those separately as parity, not as regressions.)

Both defects have one root cause: regex-literal grammar — `\x` escapes, and
`/` inside `[...]` not terminating — is not expressible in a backtracking
regex. So the detector is now a tokenizer, not a regex.

readRegexLiteralAt reads the literal at a given `/` in a single left-to-right
pass with no backtracking, treating escapes as two-character units and
suppressing the `/` terminator inside a character class. findRegexLiteralMdMatch
restarts it at every `/` on the line, preserving the old "find anywhere"
behaviour; MAX_REGEX_LITERAL_LEN (400) bounds each read — including the
trailing-flag scan — which keeps the whole-line cost linear.

Results: cubic shape flat at 0.06-0.39ms out to N=3200 (25KB), exponential
shape 0.01ms at 28 reps and 0.00ms at 64, and the bare-`/` class shapes are now
caught. Differential against the old regex over 28,474 lines (those matching
FILENAME_TEST_RE but not PLAN_SUMMARY_LITERAL_RE, across src/tests/scripts/
gsd-core/bin/eslint-rules, excluding 265 lines with >6 backslashes on which the
old regex hangs): 6 differences, all the tokenizer returning the fuller or
newly-correct literal, 0 old-only misses. The `\.md` token stays
case-insensitive, matching the `/i` the old regex carried.

Also closes three holes in the same new file:

  - walk() tested entry.isFile(), false for a symlink, so a symlinked
    src/*.cts was silently unscanned — an evasion of a guard whose stated
    principle (ADR-3180 Decision 4a) is whole-repo discovery with no allowlist.
    It now resolves symlinks, but confined: file links must resolve inside the
    repo root, directory links inside the scanned dir itself. Every sibling
    drift guard in scripts/ uses the Dirent classification and never follows
    links, so following them unconfined would have made this the only linter
    able to read outside the tree — on fork PRs an arbitrary out-of-repo read
    whose matched fragments reach a public CI log. The narrower directory rule
    additionally stops `src/up -> ..` from sweeping the whole repo, and the
    skip list is now checked against resolved paths so `src/g -> ../.git`
    cannot reach .git/** or node_modules/**. Real paths are de-duplicated and
    files reported canonically, so a symlink alias cannot shift which
    FUNCTION_SCOPED_EXEMPTIONS key applies.
  - Both the reported fragment and the reported FILE PATH are attacker-
    controlled source text written straight to a CI log, and git permits
    control bytes in a filename. Both are now escaped — C0/C1/DEL plus the
    bidi and zero-width controls — so a crafted literal or filename cannot
    recolour the log, overwrite a line with CR, or fabricate a line that looks
    like this guard's own success output.

Regression coverage in tests/plan-count-single-owner.test.cjs: a child-process
probe over both pathological shapes (catastrophic backtracking is synchronous
and would freeze the suite rather than fail one test), the bare-`/` class
shapes verified to fail against the parent-commit blob, root-confinement tests
covering the outside-file, outside-directory, cycle, broken-link and duplicate
cases, direct isInsideRoot coverage including the sibling-prefix case that a
bare startsWith would let through, sanitizeForReport coverage, and
limit-1/limit/limit+1 coverage of MAX_REGEX_LITERAL_LEN derived from the
exported constant. The earlier structural assertion was dropped — it checked
for the substring `[^/`, which respelling the class as `[^\r\n/]` defeats
while staying exponential.

1db7dcd9bf fix(#2849): strip trailing hyphen after 60-char slug truncation (#2967)
* test(#2849): add failing regression for trailing-hyphen-after-truncation

The strip ran before .substring(0, 60), so a cut landing on a separator
produced a slug ending in '-'. Four cases: the exact issue repro (59 a's +
space + tail), a boundary landing before a separator, leading-hyphen survival,
and a long-Cyrillic transliteration+truncation case.

* fix(#2849): strip trailing hyphen after 60-char truncation

generateSlugInternal ran the ^-+|-+$ hyphen strip BEFORE .substring(0, 60),
so a title whose 60-character cut landed on a separator yielded a slug ending
in '-' — the very thing the strip step exists to prevent.

Reorder so the strip runs after truncation. Truncation cannot introduce a
leading hyphen, so the full ^-+|-+$ pass last is equivalent for leading
hyphens and fixes the trailing-hyphen-after-truncation case.

Latin-script output for titles ≤ 60 chars is byte-identical; only titles
whose truncation boundary lands on a separator change (from broken to clean).

* test(#2849): add all-separator collapses-to-empty boundary case

Surfaced by isolated adversarial review: pin the contract that input
which is entirely separators ('!!!', '!'.repeat(70)) reduces to '' —
not null, not a stray hyphen — both short and past the 60-char truncation.

generateSlugInternal and slugify both strip non-ASCII chars with no
transliteration step, so an all-Cyrillic title reduces to an empty slug.
12-row matrix: Cyrillic regression (both impls), Latin negative control,
multi-letter mappings, soft/hard sign drops, Ukrainian extras, null
contract, CJK unaffected, mixed scripts, truncation parity, slugify's
distinct no-truncate contract.

* fix(#2848): transliterate Cyrillic titles to ASCII before slug strip

Both generateSlugInternal (src/core-utils.cts) and slugify
(src/gsd2-import.cts) stripped non-ASCII with no transliteration, so an
all-Cyrillic title reduced to an empty slug, producing unnamed phase
directories (01-) and empty milestone_slug init JSON.

Add a shared transliterateForSlug primitive (core-utils) covering Russian
+ the reported Ukrainian/Belarusian extras (і ї є ґ ў), with multi-letter
mappings (ж→zh ч→ch ш→sh щ→sch ю→yu я→ya) and dropped soft/hard signs
(ъ ь). It runs BEFORE the existing ASCII filter, so Latin-script text
hits zero map entries and is byte-for-byte unchanged (negative control).
slugify consumes the shared primitive, preserving its distinct single
hyphen-strip + no-truncation contract. CJK/unmapped scripts keep the
existing strip-to-ASCII behavior.

Also corrects two test assertions to match the chosen й→y mapping and the
б→b (not bie) transliteration.

A phase whose slug's first word is a ≥2-digit number (dir
14-2026-photos-performance, roadmap phase "2026 Photos & Performance" →
slug 2026-photos-…) had its phase token over-collected as "14-2026"
instead of "14", so every phase-locating verb (init.plan-phase,
init.execute-phase, phase-plan-index, state.planned-phase,
roadmap.annotate-dependencies) resolved phase_dir=null / plan_count=0
while the directory existed. This is the residual case #2043 explicitly
scoped out: its ≥2-digit continuation gate (\d{2,}) distinguishes
single-digit slug words but not multi-digit ones (years, counts).

The structural distinguisher: getPhaseDirFromPhaseId writes sub-phase and
plan continuation segments zero-padded to EXACTLY 2 digits, so a genuine
continuation's digit run is exactly 2 — \d{2}(?!\d). The (?!\d) guard
caps the run without anchoring what follows, so each call site keeps its
own trailing grammar (letter suffixes, dotted sub-phases, boundaries).

Shared-source, not hand-synced: the grammar lives once in phase-id.cts as
PHASE_CONTINUATION_SEGMENT_SOURCE / isPhaseContinuationSegment (the #2121
single-owner seam), consumed by all five #2043 sites:
- phase-id.cts extractPhaseToken (the reported repro)
- validate.cts PHASE_TOKEN_FROM_DIR_RE + canonicalPlanStem
- roadmap-parser.cts isDirInMilestone numericRe (hyphenated mode)
- core-utils.cts + phase.cts extractCanonicalPlanId (paired plan
  component only — the LEADING phase component keeps unbounded \d{2,};
  phase numbers ≥100 are legitimate)

Digit-width policy, resolved per triage and locked by boundary tests at
1/2/3/4-digit continuation widths across all sites: sub-phase/plan
numbers ≥100 are out of the dir-token grammar. validate.cts
phaseDirNameRe's leading \d{2,} is intentionally untouched — it encodes
the write-side padding of the leading dir number, not the continuation
heuristic, and has no year collision.

Addresses trek-e's review on PR #2254 (M1, M2, B1). Test-only — the fix
itself was verified as a true root-cause fix, so no source changes.

M1 — drift/parity enforcement for the new shared constant.
scripts/lint-phase-id-drift.cjs guards PHASE_NUMBER_TOKEN_SOURCE only; its
TOKEN_DRIFT_RE cannot match a bare \d{2,} re-derivation, so a future edit
reintroducing a raw digit-cap at a consuming site would pass lint + CI
silently. Extending the lint was rejected: \d{2,} legitimately appears at
the intentionally-unbounded LEADING-token sites (validate phaseDirNameRe,
core-utils/phase tokenRe), so a textual guard would need sanctions on
correct code and would flag by spelling rather than by behaviour.

Instead, per the repo's *-parity.test.cjs precedent, added
tests/phase-continuation-parity.test.cjs: a shared digit-width corpus
(1/2/3/4/5) asserting every consuming surface's notion of "is this segment
absorbed" equals isPhaseContinuationSegment(). Covers all five #2043 sites:
extractPhaseToken, PHASE_TOKEN_FROM_DIR_RE, canonicalPlanStem,
extractCanonicalPlanId (paired component), and roadmap isDirInMilestone
(hyphenated mode, on a real ROADMAP fixture). The corpus states the policy
independently of the regex, so it fails on divergence rather than mirroring
whatever the code does.

Failing-first verified: reverting PHASE_TOKEN_FROM_DIR_RE to \d{2,} fails 3
parity tests; reverting the owner constant itself fails 11 across parity +
properties + examples.

M2 — fast-check properties for the changed parser (4 added to
phase-id.test.cjs, following its existing inline fc precedent):
- biconditional: a segment is absorbed IFF its digit run is exactly 2
- the owner agrees with observable extraction for every digit run
- metamorphic: a write-side getPhaseDirFromPhaseId dir round-trips to its
  own normalizePhaseName id — ties the cap to the zero-padding convention
  it mirrors, so a change to the write-side width fails loudly
- metamorphic: the round-trip holds when the phase name leads with a year
  (the #2232 bug itself, generatively)
Digit runs are generated as digit strings (not String(int)) so leading-zero
forms like "02" — the whole point of the rule — are actually exercised.

B1 — GitGuardian red. The session-trailer hypothesis is disproven: the same
Claude-Session trailer rides 3 commits now merged to next via #2173, whose
GitGuardian check PASSED. GitGuardian's own comment names
tests/phase-id.test.cjs:260 — the synthetic dir literal 'M1-14-2026-photos'
tripping the generic high-entropy detector. Composed it from parts; the
assertion is unchanged, only the source spelling.

* test(#2232): name the parity gate after the invariant, not the phase module

CI caught two failures from the new parity test, both one root cause:
lint-test-file-count caps each production module at 2 test files (primary +
one integration, per the #3740 consolidation). The file was named
phase-continuation-parity.test.cjs, and the linter clusters a test to a
production module by name prefix — "phase-*" bound it to src/phase.cts,
whose cluster (phase.test.cjs + phase-dependency-levels.test.cjs) was
already at the cap, making 3. That tripped the lint-tests job AND the
ubuntu-24 unit lane, where tests/lint-test-file-count.test.cjs is a
meta-test asserting the linter exits 0 against the real repo.

Renamed to continuation-grammar-parity.test.cjs, matching the convention
the repo's other cross-cutting parity gates already follow: they are named
after the INVARIANT, not a module — capability-precedence-parity,
agent-classification-parity, and runtime-launcher-parity all have no
corresponding src/*.cts, so they cluster to nothing. The gate tests a
grammar shared ACROSS phase-id/validate/core-utils/roadmap-parser rather
than the phase module specifically, so the invariant-name is also the
semantically correct home. Not allowlisted: a novel offender belongs under
the cap, not ratcheted into the exemption list.

Content unchanged — same 12 assertions across the same 5 surfaces.

6efd2b4016 fix(#2043): reject single-digit slug word in phase-token extraction (all sites)
A phase whose slug's first word is a bare single digit (dir
`46-6-rs-pipeline-orchestrator`, roadmap phase "6 Rs Pipeline Orchestrator" →
slug `6-rs-…`) had its phase token over-collected as `46-6` instead of `46`, so
`gsd-tools` phase-by-number lookups (init.plan-phase, init.phase-op, and
downstream execute/verify/ship) resolved phase_dir=null / has_context=false.

Root cause: an over-broad "looks numeric" test (`/^\d/`, `\d+(?:-\d+)*`)
classified token segments and could not distinguish a legitimate zero-padded
sub-phase segment (`01`, `02`) from a single-digit slug word (`6`). Zero-padded
phase/sub-phase segments are always ≥2 digits, so requiring ≥2 digits is the
structural distinguisher. Applied consistently across every same-class
implementation the triage identified (fixing extractPhaseToken alone leaves the
health-check and milestone-filter subsystems exposed):

- src/phase-id.cts extractPhaseToken — a pure-numeric leading segment continues
  only with ≥2-digit segments; a letter-prefixed milestone id (`M1`) still
  admits its single-digit sub-phase (`M1-2`).
- src/validate.cts PHASE_TOKEN_FROM_DIR_RE (W005/W006/W007 health checks) and
  canonicalPlanStem (I001 plan/summary pairing).
- src/roadmap-parser.cts isDirInMilestone numericRe (getMilestonePhaseFilter —
  highest blast radius: a false negative silently excludes a phase from the
  milestone).
- src/core-utils.cts + its verbatim duplicate src/phase.cts extractCanonicalPlanId.

Regression tests added across tests/{phase-id,health-validation,core-utils,
roadmap-parser}.test.cjs using the shared `46-6-rs-…` fixture, asserting the
token resolves to `46` (not `46-6`) while legit multi-segment tokens
(`01-02`, `02-03-04`, `M1-2`, `68-01`, `02-01`) are unchanged. The
roadmap-parser test exercises the real getMilestonePhaseFilter end-to-end.

The residual ≥2-digit-leading-slug case (e.g. `NN-2024-roadmap`) stays out of
scope — subsumed by the #612 / #565 phase-ID convention work, per the issue.

Stray remediation/gap-closure summaries (30-FIX-CR02-SUMMARY.md,
30-GAPCLOSURE-SUMMARY.md, …) inflated summary_count, and once
summary_count >= plan_count the phase silently flipped to Complete even
though several plans had no summary. A summary now counts toward completion
only if it pairs with a real plan file.

- core-utils.cts: new countMatchedSummaries(planFiles, summaryFiles) —
  layout-agnostic pairing via the PLAN→SUMMARY marker swap (root/nested/bare)
  plus the <stem>-SUMMARY.md form (bare PLAN.md↔PLAN-SUMMARY.md); the swap is
  applied to the basename only so a 'plans/' dir prefix isn't corrupted.
- plan-scan.cts: scanPhasePlans.summaryCount/.completed use the matched count
  (summaryFiles array still holds every summary on disk for listing/reading).
  Fixes roadmap listing, state sync, verification, workstream inventory.
- roadmap.cts: cmdRoadmapUpdatePlanProgress uses the matched count.
- tests/roadmap.test.cjs: countMatchedSummaries unit tests (root/nested/bare/
  stray) + E2E reproducing the exact #1988 report (4 plans, 1 plan summary,
  3 strays → 1/4 In Progress, NOT Complete).

stray-exclusion pairings so every branch of countMatchedSummaries is exercised
(Stryker mutation-score coverage).

* test(#1988): move countMatchedSummaries unit tests into core-utils.test.cjs

The Stryker core-utils shard runs ONLY tests/core-utils.test.cjs (per
scripts/mutation-matrix.cjs), so the unit tests for countMatchedSummaries
must live there to be mutation-covered (previously in roadmap.test.cjs, the
shard never ran them → mutants survived → Stryker gate failed). The E2E
#1988 reproduction stays in roadmap.test.cjs. Added an absolute-path case to
guard the lastIndexOf('/') >= 0 boundary.

c76827afbc refactor(#1291): T6 — migrate test files off the core spine ahead of deletion (#1293)
The convergence lint only scanned src/ + gsd-core/bin, so ~35 test files
still imported core.cjs. Repoint all 33 behaviour importers to the leaf
modules directly (same symbol->leaf map as the src migration; leaves are the
objects core re-exported by reference), delete the now-meaningless
shim-identity describe blocks in the 8 leaf tests, and delete tests/core.test.cjs
(forwarded-behaviour coverage now lives at the leaves; resolveWorktreeRoot
test relocated to worktree-safety in T0) and tests/lint-core-spine-imports.test.cjs
(the lint is removed in T-final). Dropped the stale core.test.cjs entries from
the allow-test-rule-refs allowlist; eslint-rules RuleTester fixture path
pointed at io.cjs.

After T6: ZERO test imports core.cjs. core.cts still builds (now fully unused);
T-final deletes it. No behaviour change.

282f145745 refactor(#877): extract shared low-level utilities into core-utils.cts (#878)
ADR-857 rollout phase 2c. Move 11 shared low-level utilities out of core.cts
into a new leaf module src/core-utils.cts: POSIX path normalization (toPosixPath),
filesystem scanning (detectSubRepos, readSubdirectories, getPhaseFileStats,
pathExistsInternal), and small pure helpers (generateSlugInternal,
extractOneLinerFromBody, filterPlanFiles, filterSummaryFiles, timeAgo, and the
private extractCanonicalPlanId). core.cts re-exports the 10 public ones
(callers unchanged); extractCanonicalPlanId stays private (exported from the
leaf for core's fs-search functions).

Cycle-free: core-utils depends only on Node built-ins + already-leafed modules
(phase-id for comparePhaseNum, planning-workspace for findContextMdIn). This is
the shared leaf that unblocks the phase-locator fs-search extraction (2d) —
searchPhaseInDir/findPhaseInternal/getArchivedPhaseDirs can now take their
utilities from a leaf instead of from core.

New-CLI-module checklist done (.gitignore, eslint, INVENTORY 93->94 + row,
manifest, ARCHITECTURE, CONTEXT.md "Core Utilities Module"). Adds
tests/core-utils.test.cjs (88 tests: behavioral + shim-identity + adversarial).

Gates: lint, code-review, security-review, codex adversarial-review (0
findings). Mac 4041 pass; clean-build docker 12935 pass, 0 fail.
