Mechanical rename produced by scripts/msd-rename.cjs: gsd/Gsd/GSD -> msd/Msd/MSD across contents and paths, upstream package/repo coordinates -> @golem15/msd-core and golem15com/msd-core. Deep links into upstream history, sibling upstream packages, the GSD-2 import feature, CHANGELOG.md and .changeset/ are kept as-is. Hand edits on top: MSD block-letter banner and logos, LICENSE copyright line, package/plugin identity, regenerated lockfile, install-tree fixtures, derived registries and benchmark baseline; migration checksum baseline re-locked (MSD keeps its own install state, so no install had applied the old sums); sort-order and regex-escaped expectations in tests adjusted. |

fix(#4658): give the ui plan gate's evidence check a native branch (#4807)
* test(#4658): add failing-first coverage for native frontend evidence hasStaticFrontendEvidence recognised only JS-ecosystem evidence, so computeUiPlanGate could never block for a SwiftUI/Compose/Flutter/XAML project. Adds evidence-level fixtures for the four suggested markers (import-matched for .swift/.kt/.dart, extension-alone for .xaml), the reporter's non-UI Swift control case, marker-exactness and SKIP_DIRS and I/O-degrade negatives, gate-level block assertions through makeProject's new native frontendEvidence modes, and a pinned-seed fast-check property. * chore(#4654): add local/no-unconfined-path-join and drain it to zero Phase 4 of epic #4636 — the ratchet, and the phase that makes the epic hold. THE MEASUREMENT THAT RESHAPED THE PHASE. An AST census (the repo's own parser, not grep) found what the epic never enumerated: ADR-4650 named seven containment implementations; `src/` alone held roughly 24 more hand-rolled gates across ~13 files, several guarding a write or an `fs.rmSync`. Two verified by reading rather than pattern-matching — `research-store.cts` comments its own as "ensure the resolved file path stays inside the store dir" immediately before a write, and `capability-lifecycle.cts` gates `fs.rmSync` with one. So the epic's Done-when "one containment predicate, used at every site" was FALSE when Phase 3 reported it satisfied. It is true now: the rule is clean across src/, scripts/, gsd-core/bin/ and hooks/ with an EMPTY allowlist. WHY NOT THE RULE THE ISSUE PROPOSED. #4654 proposed flagging `path.join` whose first argument is a managed root and whose later arguments derive from argv. That is a taint analysis over 2046 call sites, in ESLint, without type information; "derives from argv" is not locally decidable. Any approximation either floods or is trivially evaded, and a rule that fires on hundreds of correct sites earns an allowlist of hundreds — the opposite of a ratchet. What is actually duplicated is the COMPARISON, not the join, and that has one recognizable shape. Arm 1 X.startsWith(Y + sep) the hand-rolled containment idiom Arm 2 a containment predicate called as a bare statement, answer discarded Arm 2 is the issue's "asserts the result was narrowed, not merely that a helper was called". Its example `validatePath(x, root).resolved` is already structurally impossible — Phase 3 un-exported `validatePath` — so the remaining expressible failure is ignoring the answer, which is the defect that recurred five times in this epic. The census found exactly one live instance (`milestone.cts:1643`); it now returns the proven `ContainedPath` so consumers stop re-deriving the path the comment above it was extracted to stop them re-deriving. The rule deliberately does NOT try to catch validate-one-path-use-another where the answer is used but a different variable flows onward. That needs flow analysis; the branded `ContainedPath` from Phase 3 is the defense there, and the two are complementary. PER-SITE FAMILY CHOICE, NOT A DEFAULT. Phase 3's lesson binds: collapsing a lexical site onto the realpath family broke four tests and was caught only by the matrix. Every migrated site was triaged individually. The six installer-migrations tree-walks and the six capability-lifecycle gates take the LEXICAL family because their operands are already realpath-resolved and they deliberately treat the final component as a link; boundary sites take realpath. TWO SITES WITH AN INVERTED CONTRACT, which a mechanical swap would have broken. `installer-migrations.cts:127` and `runtime-artifact-install-plan.cts:144` REJECT `target === root` by contract, while the canonical comparison ACCEPTS it. Swapped naively, a migration could `rmdir` the user's config root and a third-party descriptor could write at configHome itself. Both keep `=== root` as an explicit additional arm alongside the predicate call — the predicate decides containment, the call site keeps its own extra condition (ADR-4650 decision 6). ONE DUPLICATE DELETED OUTRIGHT: `planning-inspect.cts`'s `isWithinRoot` was byte-identical to `isContainedIn` and said so in its own docstring. `isContainedIn` is now exported for callers that have already resolved both operands and need only the comparison, with a doc note that a caller which has NOT resolved them must use a full predicate instead. THE MARKER, AND WHY IT IS NOT THE ALLOWLIST. Nine sites are justified holdouts and carry `// allow-handrolled-containment: <reason>` with a mandatory, reviewable reason. Two justifications: (a) not a containment decision — an ancestor-walk loop condition, sub-repo grouping, worktree identity matching, declared-path coverage; (b) it IS containment but the canonical predicate is unreachable — `capability-validator.cjs` is a committed pre-build `.cjs` and the compiled `security.cjs` is untracked build output, so requiring it would break a fresh clone. `scripts/lib/drift-scan.cjs` runs under `lint:ci` with the same exposure. The marker was renamed from `allow-lexical-prefix-match` mid-phase because that name asserted only (a) and would have stated something false at the (b) sites. A marker suppresses BEFORE the violation counter increments, so a file whose every occurrence is marked still reports `staleAllowlistEntry` — otherwise a drained entry lingers and silently re-permits the site later. DEMONSTRATED RED, per #4654: a hand-rolled copy reintroduced into a real `src/` file made `npm run lint` fail with the rule's full guidance message; removing it returned the tree to clean. Both halves recorded — red alone proves nothing, since a rule red for an unrelated reason looks identical. DISCLOSED: `defaultRequireFromInstallRoot` (gsd-tools.cjs) previously carried two distinct rejection messages and two manual realpath calls; routing it through `tryWithinRoot` collapses them to one message, and a missing module now surfaces as MODULE_NOT_FOUND rather than ENOENT. No test asserts either message. The security property is preserved and slightly strengthened — the candidate is realpathed and containment re-checked, and the dangling-symlink oracle closure comes along with it. I searched tests/ for the literal string "resolves outside its install root", found nothing, and reported that no test asserted it. The test matches a REGEX SUBSTRING, /outside its install root/, so the literal search missed it. What broke was "NEGATIVE PROOF: a symlinked module pointing OUTSIDE the install root is not loaded" — the test guarding the exact property I claimed was preserved. defaultRequireFromInstallRoot now does both checks again with both messages byte-identical, each routed through the canonical predicate, which is better than the original since that hand-rolled both comparisons. MATRIX FAILURE 2 — shipped migrations are checksum-locked, and a marker cannot serve there. migrationChecksum hashes plan.toString(), which INCLUDES comments, so a suppression marker inside a plan body drifts the baseline exactly as an edit does. Measured: with markers in place, two of the four still differed from their committed checksums. The four shipped bodies are now byte-identical to next, and the rule's config excludes those four paths BY NAME rather than by a directory wildcard, so a NEW migration is still covered. Six containment comparisons stay un-ratcheted there; that gap is recorded in the rule's Known gaps, in CONTEXT.md and in the security model rather than left implicit. Justification (c) is removed from the marker's documented reasons, because a marker was proven unable to express it. ADVERSARIAL REVIEW — the sharpest finding was that the rule banned the CORRECT shape while permitting the incorrect one: startsWith(root) with no separator is the genuinely unsafe form, since it accepts a sibling such as root-evil, and my own test blessed it as valid. Flagging every bare startsWith would swamp the rule, so that stays a STATED gap rather than a silent one. Closed for real: the template-literal spelling, which the census never saw because it only inspected plus-concatenation — that surfaced TWELVE more sites, now triaged and migrated. A separator reached through a const alias is now resolved via scope analysis. And isContainedIn, exported in Phase 3, was missing from the discarded-result set, so a bare no-op call went unflagged on the one function the epic funnels through. SECURITY REVIEW — the marker could over-suppress two ways: a block comment worked identically to a line comment, and one marker silently covered every violation sharing its line. It now requires a Line comment positioned after the flagged node ends, so it anchors to the node it trails. Four sites had dropped an unreachable-but-deliberate equality rejection against the root; each is restored as the call site's own arm. eslint.config.mjs still documented the OLD marker token, which my rename missed — it would have sent the next author in circles. A FALSE GREEN, recorded because it nearly stuck: lint:ci reported exit 0 from a stale eslint cache while twelve real violations existed. Every lint check here now clears the cache first. The case "two violations on one line: trailing marker suppresses only the one it trails" expected 1 error and got 0 — both were suppressed. ROOT CAUSE: the anchoring accepted any Line comment on the node's line whose range started at or after the node's end. A trailing marker at the END of a line sits after EVERY node on that line, so that condition held for all of them. "After the node" does not identify WHICH node the marker trails. The fix reads as correct and is not. FIX: deferred reporting. Violations accumulate during traversal instead of being reported immediately; at Program:exit each marker claims exactly ONE pending violation — the one on its line whose end is nearest before the marker begins — and every unclaimed violation is then counted and reported. One marker, one suppression. An earlier violation sharing the line is still reported, which is the property the security review asked for and the previous attempt only appeared to deliver. The counter now increments at flush time rather than during traversal, so a suppressed occurrence still does not keep an allowlist entry alive. AND A TOOL THAT SHOULD HAVE EXISTED BEFORE THE FIRST MATRIX RUN. `node --test` is hard-blocked here, so this rule's test file could only ever be executed on the remote matrix — which is why a broken anchoring shipped into a run. ESLint's programmatic Linter API is not a test runner, and exercising the rule through it verifies every case locally in seconds. All 24 now pass locally, including the two-on-one-line case that failed remotely. That loop should have been built before the rule was first sent to the matrix rather than after it failed twice. A docs/how-to page would be a second, driftable copy read by nobody at the moment of failure. enablementSequence is recorded as what it actually is: a VERIFICATION sequence, not an enablement one. The rule is never off, so there is no off-to-on transition to describe. scripts/lint-docs-required.cjs now passes (ok_docs_updated) — it could not evaluate against the mandated pr:0 placeholder. refactor(#4653): replace the allowAbsolute flag with a named acceptance policy
Phase 3 of epic #4636, stage 3b. Satisfies #4653's criterion that `opts.allowAbsolute` become "a named acceptance policy on the predicate, not a per-call-site boolean". The flag was actively misleading at the call site. `{ allowAbsolute: true }` reads as "containment is relaxed here". It never was: an absolute path that resolves outside the root is rejected exactly as a traversal is. The flag only ever controlled whether an absolute candidate was CONSIDERED. On a security predicate that is the wrong thing for a reviewer to have to infer, and 31 call sites were asking them to infer it. PathAcceptance.RelativeOnly relative candidates only PathAcceptance.AbsoluteInsideRoot absolute accepted, containment unchanged The three exported wrappers take the policy and translate it inward. validatePath keeps its internal `{ allowAbsolute }` opts and its body untouched — the engine is not re-derived here either, only the exported surface is renamed. MEASURED, NOT ESTIMATED. 31 call sites across 10 files, counted by walking the AST with the repo's own @typescript-eslint/parser rather than grepping: a text match would have folded in the options-type declaration, default parameter values and comments. All 31 pass the literal `true`; none passes `false` or a dynamic value, so the migration is uniform and `RelativeOnly` is purely the existing default made nameable. audit.cts alone holds 18 of them. This migration is compiler-verified in a way the containment-value migration in the previous commit was not: the parameter type changed from an object to a string union, so any missed site is a build error rather than a silent behavioral difference. refactor(#4653): narrow the containment export and migrate all 15 validatePath sites
Phase 3 of epic #4636, stages 1-2. Implements ADR-4650 decisions 1 and 2: one containment predicate, and an exported shape that cannot hand a caller a usable path when the answer is unsafe. THE EXPORT IS NOW A PAIR, BOTH RETURNING A BRANDED TYPE: assertWithinRoot(candidate, root, label?, opts?) -> ContainedPath (throws) tryWithinRoot(candidate, root, opts?) -> ContainedPath | null ADR-4650 names only the throwing form. That does not survive contact with the call sites: findPhaseArtifact probes a direct path, then a .planning/ path, then each readdir entry, and throwing on the first miss breaks it outright. Six of the fifteen sites need a non-throwing check. Recorded here rather than papered over. WHY BRANDED. validatePath returns { safe, resolved, error } and populates resolved with the ESCAPING path on the traversal branch — so a caller who skips the boolean gets an attacker-controlled value precisely in the dangerous case. tryWithinRoot returns exactly null there; assertWithinRoot throws. A plain string is not assignable to ContainedPath, so a migrated site that validates one path and then passes a different one is now a type error rather than a silent bug. That is the defect this epic exists to close, and I introduced it twice in Phase 2. THE ENGINE IS UNTOUCHED. validatePath's body is not re-derived — the diff shows zero edits to the dangling-symlink existence-oracle closure, the ancestor canonicalization (macOS /var vs /private/var), or the separator-aware boundary test. Each was acquired as a bug fix and a re-derivation would silently lose one. requireSafePath now delegates to assertWithinRoot, so there is one implementation beneath both names; its return type is branded, which is why its 13 call sites compile unchanged. A TYPESCRIPT LIMITATION, FIXED AT THE ROOT RATHER THAN WORKED AROUND. TS applies never-return control-flow narrowing only when the callee is a function declaration or a const with an EXPLICIT type annotation. Both routers do `const { error } = io` — destructured, unannotated — so `error(...)` did not narrow ContainedPath | null and four sites wanted a dead `throw new Error('unreachable')` after it. Annotating the const (`const error: typeof io.error = io.error`) makes TS narrow properly and the dead throws are gone. That annotation has a large, deliberate consequence: with narrowing working, `@typescript-eslint/no-unnecessary-type-assertion` fires at 37 sites in commands.cts where `as string` / `!` existed ONLY to paper over the missing narrowing. They are removed. The rule is type-aware and fires only where the assertion changes nothing, and both forms erase at compile time, so the emitted behavior is unchanged — but the module loses 37 unchecked casts over string | undefined, which is the same class of "trust me" the containment work is removing. Widening the diff here buys that. TWO SITES LOSE DIAGNOSTIC TEXT, deliberately. tryWithinRoot has no error channel, so init.cts's agent-skills warning and cmdPrSubrepo's rejection now name the condition rather than echoing validatePath's message. verify.cts had already stopped echoing it on purpose — the message embeds absolute host paths — so this makes the three agree instead of two-of-three. fix(#4652): use the validated path, collapse the duplication, correct two false claims
Seven findings from the two-axis review, all fixed in place. THE ONE THAT MATTERS: cmdTodoComplete validated sourcePath and targetPath and then ran every fs call against the RAW strings — existsSync, statSync, readFileSync, platformWriteSync, unlinkSync, and the dry-run path payload — never sourceCheck.resolved / targetCheck.resolved. That is the exact "validate one path, use another" shape ADR-4650 names as the defect this epic exists to prevent, and it is the same bug this phase had just fixed in check-command-router. Committed inside the fix for it. All I/O now uses the resolved paths; user-facing messages still echo the raw filename, never a resolved absolute path. A VACUOUS TEST, and the false doc claim it was propping up. The test "[RED #4327] an absolute path outside the project is rejected" would have passed with ZERO containment logic: path.join(pendingDir, '/abs/outside/x') yields <pendingDir>/abs/outside/x — Node does not let a later absolute segment escape — so the name is FOLDED under the root, passes containment, and simply 404s. The test only ever observed "Todo not found". It now asserts what is actually true and actually valuable: an absolute name is neutralized, and the real outside file is not read, not moved, and still present afterward. docs/CLI-TOOLS.md claimed such a path "is rejected as a usage error", which was false; it now describes the fold-under-root behavior. Traversal and embedded separators ARE rejected, and those claims stand. DUPLICATION THIS EPIC EXISTS TO REMOVE. resolvePath already did isAbsolute-or-join + validatePath + reject; cmdGapAnalysisPlanPost and cmdCheckPredicate each re-inlined the identical triplet in the same file. Both now call resolvePath. Cost, stated rather than hidden: its generic message replaces the two sites' distinct "phase-dir escapes…" wording. The message still names the offending input, and one predicate with one message is the point. SYMLINK COVERAGE was required by #4652's "Done when" and was missing. Added for both the todos root and --phase-dir, skipping cleanly on EPERM so the Windows lanes do not fail where unprivileged symlink creation is disallowed. Both fast-check properties were UNSEEDED. Seeded now. The changeset named "check decision-coverage-plan" as a boundary; that is a caller of the shared resolvePath, which the body never mentioned. Corrected. DISCLOSED, not hidden: ctx.phaseDir is now always the resolved ABSOLUTE path, so ${PHASE_DIR} interpolation and the "not found in <targetDir>" message show an absolute value where a relative --phase-dir previously produced a relative one. That is an observable output change. A test pins it and docs/reference/gate-predicates.md states it. Also regenerated scripts/lib/platform-conformance-tier.generated.cjs and its macos twin — the new tests changed check-predicate.test.cjs's tier classification. fix(#4652): confine every boundary that joins argv to a managed root
Phase 2 of epic #4636, absorbing #4327 and #4354. Implements ADR-4650 decision 3: containment is a boundary concern — the predicate runs where external input enters, not at whichever interior call site remembered. Four boundaries now validate against their managed root and reject with a USAGE-shaped error before touching the filesystem: todo complete <name> -> todosDir(cwd) check predicate --phase-dir <dir> -> projectDir check decision-coverage-plan <dir> -> projectDir (via resolvePath) check gap-analysis.plan-post <dir> -> projectDir #4327 understated its own severity. It reports that a traversal name "resolves outside the todos root", which reads as an information leak. Measured, it was destructive: the command exited 0, MOVED the outside file into completed/, and unlinked the original. cmdTodoComplete ends in fs.unlinkSync(sourcePath), so an unconfined name consumed across the boundary rather than merely reading across it. Validation now precedes every fs call — existsSync, readFileSync, ensureDir, writeSync, unlinkSync — and both halves of the move are confined, so neither source nor destination can land outside the root. --dry-run is rejected on the same terms; a preview must not leak a resolved outside path either. #4354 reproduces exactly: a BLOCKING gate returned block:false sourced entirely from a SECURITY.md in a caller-chosen directory outside the project. THE HARDER HALF, found by the isolated adversarial review of the first attempt: validating a path and then using a DIFFERENT one closes nothing. The first fix validated `--phase-dir` joined against `--cwd`, then passed the RAW unjoined value into the predicate context. gate-predicate-evaluator uses it as-is and findPhaseArtifact resolves a relative path against the REAL process cwd — so validation and the read used two different roots whenever process.cwd() differed from --cwd. Reproduced: running from a directory holding a plan with `secret_field: LEAKED_VALUE`, a predicate declared against an empty --cwd project exited 0 and returned "actual":"LEAKED_VALUE". Confining in the router is what keeps that pure-leaf contract intact AND covers ${PHASE_DIR} interpolation into command-exit-zero, which an evaluator-local fix would have missed entirely. Also fixed, same review: `todo complete .` and `..` passed containment (they resolve to the pending dir, which IS inside the root) and then threw an uncaught EISDIR with an absolute-path stack trace. Now a clean USAGE rejection naming the real reason — "todo name is not a file" — rather than borrowing the escape message, which would have stated something false. Also adds the seeded property the test matrix had promised: for any plan body, the scope read back is whitespace-only exactly when the body was. That pins the gate's discriminator to bytes examined, so it cannot quietly become "no signals found", across unicode whitespace and CRLF. `docs/INVENTORY.md` picks up the reference doc's new seal-time outcome table — surfaced by the co-change gate, not by a lint failure. Shipped content now emits an ATX heading for a titled section and a blank-line-delimited --- for a break between sections, both of which adapt to the available width. The same convention is applied to the three code sites that built these strings at runtime: the UAT checkpoint renderer, the milestone-close audit report, and the TDD review checkpoint table. Removing the box also removes its only reason to exist -- the east-asian-width padding helpers that kept its right border aligned (checkpointBoxLine, displayWidth, isWideCodePoint, ZERO_WIDTH_MARK_RE, CHECKPOINT_BOX_WIDTH). * refactor(#3183): route live-plan counting through scanPhasePlans scanPhasePlans becomes the sole owner of the live-plan derivation. Twenty-one independent re-derivations across seven modules now route through it, and scripts/lint-plan-count-drift.cjs reports zero, scanning the whole repo rather than an allowlist (ADR-3180 Decision 4a). The epic scoped this at three copies. A whole-repo guard found twenty-six sites across nine files, so Phase 1 absorbs every live-plan re-derivation and Phase 3 narrows to window plus sentinel enumeration. Two sites are exempt with a documented reason rather than a bare allowlist: audit.cts scans one quick task's own directory for a single completion record, and gsd2-import.cts reads a foreign GSD-2 tasks/ layout during a one-time import. Neither is a phase directory. scanPhasePlans gains allPlanFiles (pre-supersession) alongside planFiles so one owner answers both questions: verify.cts's numbering-gap check wants every plan on disk, its pairing check wants the live set. Both fields are additive. Highest-severity fix: cmdPhasePlanIndex, which feeds execute-phase wave scheduling, was scheduling status:superseded plans into waves and reporting zero plans for the post-#3139 nested layout. filterPlanFiles and filterSummaryFiles are deleted; getPhaseFileStats orphaned them and only their own tests still called them. New leaf module src/planning-scope.cts carries the frozen SCOPE discriminator, with its six-gate ripple closed: gitignore, inventory manifest, INVENTORY.md and the CONTEXT.md glossary. This guard is the last stage of `npm run lint:ci`, which CI runs on fork pull requests, so a crafted src/*.cts could stall the job. - A DETECTION HOLE. A character class holding a bare, unescaped `/` — e.g. `/SUMMARY[^/]*\.md$/`, an ordinary path-excluding filter — terminated the literal at that `/`, so the scan never reached `\.md` and the guard missed it entirely. (Classes holding an ESCAPED `\/` were already matched; the tests cover those separately as parity, not as regressions.) Both defects have one root cause: regex-literal grammar — `\x` escapes, and `/` inside `[...]` not terminating — is not expressible in a backtracking regex. So the detector is now a tokenizer, not a regex. readRegexLiteralAt reads the literal at a given `/` in a single left-to-right pass with no backtracking, treating escapes as two-character units and suppressing the `/` terminator inside a character class. findRegexLiteralMdMatch restarts it at every `/` on the line, preserving the old "find anywhere" behaviour; MAX_REGEX_LITERAL_LEN (400) bounds each read — including the trailing-flag scan — which keeps the whole-line cost linear. Results: cubic shape flat at 0.06-0.39ms out to N=3200 (25KB), exponential shape 0.01ms at 28 reps and 0.00ms at 64, and the bare-`/` class shapes are now caught. Differential against the old regex over 28,474 lines (those matching FILENAME_TEST_RE but not PLAN_SUMMARY_LITERAL_RE, across src/tests/scripts/ gsd-core/bin/eslint-rules, excluding 265 lines with >6 backslashes on which the old regex hangs): 6 differences, all the tokenizer returning the fuller or newly-correct literal, 0 old-only misses. The `\.md` token stays case-insensitive, matching the `/i` the old regex carried. Also closes three holes in the same new file: - walk() tested entry.isFile(), false for a symlink, so a symlinked src/*.cts was silently unscanned — an evasion of a guard whose stated principle (ADR-3180 Decision 4a) is whole-repo discovery with no allowlist. It now resolves symlinks, but confined: file links must resolve inside the repo root, directory links inside the scanned dir itself. Every sibling drift guard in scripts/ uses the Dirent classification and never follows links, so following them unconfined would have made this the only linter able to read outside the tree — on fork PRs an arbitrary out-of-repo read whose matched fragments reach a public CI log. The narrower directory rule additionally stops `src/up -> ..` from sweeping the whole repo, and the skip list is now checked against resolved paths so `src/g -> ../.git` cannot reach .git/** or node_modules/**. Real paths are de-duplicated and files reported canonically, so a symlink alias cannot shift which FUNCTION_SCOPED_EXEMPTIONS key applies. - Both the reported fragment and the reported FILE PATH are attacker- controlled source text written straight to a CI log, and git permits control bytes in a filename. Both are now escaped — C0/C1/DEL plus the bidi and zero-width controls — so a crafted literal or filename cannot recolour the log, overwrite a line with CR, or fabricate a line that looks like this guard's own success output. Regression coverage in tests/plan-count-single-owner.test.cjs: a child-process probe over both pathological shapes (catastrophic backtracking is synchronous and would freeze the suite rather than fail one test), the bare-`/` class shapes verified to fail against the parent-commit blob, root-confinement tests covering the outside-file, outside-directory, cycle, broken-link and duplicate cases, direct isInsideRoot coverage including the sibling-prefix case that a bare startsWith would let through, sanitizeForReport coverage, and limit-1/limit/limit+1 coverage of MAX_REGEX_LITERAL_LEN derived from the exported constant. The earlier structural assertion was dropped — it checked for the substring `[^/`, which respelling the class as `[^\r\n/]` defeats while staying exponential. test(#3056): add the in-process fault-injection adapter, and normalize execGit's shape (#3077)
* refactor(#3071): normalize execGit's call and result shape, unify ExecGitFn ExecGitFn was declared four times. Three were hand-copies of one signature and two of those were wrong: they typed exitCode as number|null when _spawnResult returns `result.status ?? 1` and can never yield null, weakened signal from NodeJS.Signals to string, and widened error from Error to unknown. Only verification.cts got it right, via `typeof execGit`. The root cause was a missing export: SpawnResultOutput was declared without `export`, so no other module could name the return type of execGit. Three authors independently hand-copied it instead. Exported now. Normalizing the type alone would have left the pressure that caused the divergence, so the function is normalized on both sides. It now ACCEPTS every call its consumers make — worktree-safety's declaration could not express an env-carrying call at all — and RETURNS every result code they need: timedOut moves into _spawnResult, so execGit, execNpm and execTool all carry it and the one extension that justified a separate type disappears. All four sites are now `typeof execGit` with nothing left to restate. timedOut reuses the existing isSpawnTimeout predicate introduced by #3050 rather than re-deriving it. That predicate checks error.code === 'ETIMEDOUT' only; the signal === 'SIGTERM' conjunct was deliberately dropped there because Windows does not reliably report SIGTERM and requiring it risks a false negative. There is no false-positive risk, and a test proves it: an externally-delivered SIGTERM leaves error null, so it is still not reported as a timeout. No dead null-checks surfaced. Every exitCode comparison in the two affected modules is === 0, !== 0 or === 128 — never a null guard — so the nullable declaration had never been written against. A parity test drives the same stub through a real injectable entry point in each of worktree-safety, git-base-branch, worktree-base-ref and verification; it fails the moment any of them re-grows its own shape. Faults are scoped rather than global — by argv predicate and by call ordinal — because a fault adapter that faults everything looks like it works and proves nothing, and because verification.cts's two-call error handling needs to fault the second call only. Invocations are recorded so a test can assert an exact call count. The timeout fault carries error.code === 'ETIMEDOUT', and a test asserts the real isSpawnTimeout predicate matches it, so the fixture cannot drift from the production definition of a timeout. A companion test asserts an externally delivered SIGTERM with a null error is still NOT reported as a timeout — the false-positive guard for #3050's dropped conjunct. withFaultyFs restores in a finally so a throwing body still restores, patches only the named methods, and nests without clobbering an outer saved original. It never uses chmod: that no-ops under root, so the test would pass with zero coverage in root Docker/CI. The adapter is in-process via deps only. The Phase 1 process seam is documented as deliberately not a fault-injection surface — it cannot distinguish an injected timeout from a genuine bench OOM and would retry it. Both fail in both directions. On Windows a genuine timeout is not reliably reported as SIGTERM, so the guard silently fails to fire — the false negative #3050 was raised for. And an externally-delivered SIGTERM is not a timeout at all, so the check also fires when it should not; isSpawnTimeout avoids that because `error` is null in that case and it keys on error.code. Both now read the derived `timedOut`, and graphify's comment states the actual rule instead of the fragile assumption. The fixture was wrong, not the production change. It stubbed spawnSync as { status: null, signal: 'SIGTERM', error: undefined }. That is not a timeout. A real spawnSync timeout also sets error.code 'ETIMEDOUT'; a SIGTERM with no error is an externally delivered signal — a kill. The old `result.signal === 'SIGTERM'` check accepted it as a timeout, which is the false positive the shared predicate exists to reject, so this test was locking that bug in rather than guarding against it. The fixture now carries a real ETIMEDOUT error and all three original assertions pass unchanged. A counter-test is added alongside it: an externally delivered SIGTERM with no error must NOT be reported as a timeout. That is the assertion whose absence let the false positive live. Swept every SIGTERM/SIGKILL stub under tests/ for the same unfaithful shape. No other instance: the worktree-safety, worktree-base-ref and commit-staging fixtures already set ETIMEDOUT, and the remaining hits are either deliberate external-kill tests or feed code that never consults timedOut. Two sites keep their own signal check deliberately and are NOT changed: capability-source.cts:1301,1386 fail closed on ANY abnormal termination, which is correct — reading timedOut there would stop it failing closed on a kill and let it parse stdout from a killed process. The advisory gate then short-circuited to a confident pass with no violations table. Adds tddPlanCrlf() fixture helper (CRLF twin of tddPlan) and a [crlf] test case that asserts tddPlans===1 for a CRLF type:tdd plan (would be 0 before the fix), plus block:true/violations:1 since the fixture has no RED/GREEN commits. * fix(#2449): make tdd-review-checkpoint frontmatter regex CRLF-tolerant The frontmatter delimiter regex at src/check-command-router.cts:751 used literal \\n which cannot match a CRLF PLAN.md delimiter (---\\r\\n). frontmatterMatch was null, the plan was never classified type:tdd, tddPlanFiles stayed empty, and the advisory gate short-circuited to a confident pass with no violations table. The advisory affects body-parser >=2.0.0 <2.3.0 pulled transitively via @anthropic-ai/claude-agent-sdk -> @modelcontextprotocol/sdk -> express -> body-parser@2.2.2. express@5.2.1 already declares body-parser as ^2.2.1, so 2.3.0 is a valid re-resolution within express's own compatibility range — no override needed. Regenerated the lockfile via 'npm audit fix --omit=dev' which re-resolves transitive deps within their declared ranges; package.json is unchanged. Verified: npm audit --omit=dev reports 0/0/0/0/0 advisories; body-parser now reads as 2.3.0 in 'npm ls body-parser --omit=dev'. * test(#2449): add mixed-endings (CRLF frontmatter + LF body) variant Per code-review Low finding: CONTRIBUTING.md:490 names 'Mixed CRLF/LF newlines' as a required adversarial fixture class. * fix(#2372): widen decision-coverage scan to planner-canonical tags, fix message Bug: check.decision-coverage-plan's remediation message told the user to cite decisions "(or body)" but extractPlanDesignatedSections only scanned <objective>/<tasks>/<task>/<action>. A decision cited in <read_first>, <behavior>, <verify>, <acceptance_criteria>, or <done> was invisible to the gate — false BLOCKING coverage gap, plus the message's own fix-hint sent the user to "the body" where re-citing still failed. Two-part fix (must change together — that drift was the bug): 1. Widen XML_DECISION_TAGS_RE in src/check-command-router.cts to also match <read_first>, <behavior>, <verify>, <acceptance_criteria>, <done>. These are all planner-canonical tags the planner is told to use (plan-phase.md:830-862, plan-phase.md:772). The body negative- lookahead mirrors the opening-tag set so each tag's body is captured independently. 2. Correct buildPlanMessage to name ONLY the surfaces the extractor actually scans (front-matter must_haves/truths/objective, designated markdown headings, and the nine planner-canonical tag bodies). The misleading "(or body)" clause is gone. Also updates the planner's documented contract (agents/gsd-planner.md:69) and user-facing docs (docs/CONFIGURATION.md, docs/USER-GUIDE.md) to reflect the wider scan. Regression tests in tests/decisions.test.cjs cover each newly-scanned tag body, a control (no citation still uncovered), and a message/extractor parity assertion that names every scanned surface — so the two cannot drift apart again. Out of scope (per triage): cmdDecisionCoverageVerify/buildVerifyMessage is a separate command (decision-coverage-verify) checking shipped artifacts, not plan citations — untouched. * chore(#2372): regenerate agent-size-baseline + golden-install-parity fixtures gsd-planner.md grew 49172 → 49294 (+122 chars) from the widened decision- coverage contract (5 new scanned tag names + heading clarification). Growth is justified: the contract surface is itself the fix — the prior text under-described what the gate scans, which was the bug. Concretely: <action>per D-05 <verify>npm test</verify></action> → 3-tag alternation (old): captured 'per D-05 <verify>npm test</verify>' as <action> body → D-05 caught → 9-tag alternation (bug): captured 'npm test' only (from <verify>); D-05 in <action> prefix LOST Switches extractXmlTagBodies to per-tag matching: each tag gets its own regex whose negative-lookahead tempers only against the SAME tag's reopening. So <verify> inside <action> is absorbed into <action>'s body (D-05 caught) AND <verify> is matched separately on its own pass. * fix(#2365): stop the api-coverage detector false-positiving non-API phases detectApiIntegration fired on any integration verb co-occurring anywhere on a line with any API noun, treated / as a word boundary (so a first-party Next.js src/app/api/... route path matched the noun "api"), and read any capitalized word before API/SDK/REST/GraphQL as a service name behind a fixed stopword denylist (so threat-model prose like "Resolver-only API" fired). Because the verify:pre seal gate is BLOCKING, a phase touching no external API could not reach UAT without fabricating a coverage matrix. The compound rule now requires the verb and noun to share one clause (sentence punctuation and table-cell walls end a clause) within a bounded word gap. Non-prose spans are excluded before matching: fenced code (already), inline code spans (new stripInlineCode in the markdown-sectionizer seam), and path-shaped tokens. The <Service> API surface rule requires proper-noun position — a clause-initial capitalized word is ordinary English and needs dependency evidence (URL / package reference) on the same line — and rejects compound modifiers ("Resolver-only", lowercase after the hyphen). A phase that integrates no external API now has a first-class, reasoned way to say so: a COVERAGE.md containing "No external API integration: <reason>" satisfies the gate (declaration + rows is contradictory and blocks). The true-positive path is pinned by regression tests: every default-vocabulary positive still fires, including the widest word-gap pairing and the surface-rule-only shape. New false positives fixed (all now clean): - F6 a URL token that swallowed a trailing clause comma, merging two clauses — trailing clause punctuation is kept literal so the split survives. - F7 a capitalized internal component authorizing cross-clause binding — the new gate requires a dependent elaboration, not a new coordinate clause opened by a conjunction ("…, then document…"). - F8 a protocol name read as a service ("REST API", "GraphQL API") — protocol and locality descriptors are rejected in the `<Service>` position. - Finding 9: the inline-code-span scanner was O(n^2) on pathological backtick runs; rewritten to linear via a per-length run cursor (2 MB: 4.15 s -> ~6 ms), semantics preserved (148 sectionizer tests unchanged). Net simplification: the fail-closed model removed the round-2 minWordGap / groupByTerm / follower / corroboration machinery (350 insertions vs 445 deletions across the touched files). Under fail-closed, three round-2 negative tests now correctly detect (integration verb + "internal"-qualified noun, and the distant-same-clause case); none were trek-e acceptance FPs. A bare domain link with no path ("https://example.com") stays a non-signal, so "Integrate … from example.com, document …" is still clean. - Locality qualification ("internal", "private") no longer leaks across a sentence or clause boundary: only plain spaces may separate the descriptor from the service, so "The cache is private. Stripe API …" now detects. - Cross-clause binding: the fragile head-word cap (which could not tell a genuine "Connect … to Stripe payments, exposing its endpoints" from an unrelated "Integrate … from URL, document …" — both 4 words after the verb) is replaced by a participial-continuation rule: a verb binds a noun in the next clause only when that clause begins with an "-ing" elaboration. This fixes the 4-word-head false negative AND the false positive below at once. False positives (now clean): - Cross-clause no longer binds a finite continuation regardless of separator: "Wire the settings form. Document endpoint props." / "…; document …" / "…, document …" are separate actions, not elaborations. Perf (quadratic → linear): - The trailing-punctuation peel is a backward char scan instead of an unanchored `[…]+$` regex (16k chars: 156 ms → ~1 ms). - SERVICE_SURFACE_API_RE bounds the service-name length {1,40} so a hostile "A-A-…-x" run cannot drive O(n^2) backtracking (16k: 385 ms → ~3 ms). Consumer fail-open (blocking gate): - readPhaseScope now distinguishes "no plans" from a plan that EXISTS but is unreadable. On a read error the gate BLOCKS ("could not read the phase scope …") instead of silently certifying no-integration from partial scope — an unreadable plan could be the one describing the integration. Documented fail-closed tradeoffs, now pinned with tests so they are not "fixed" back into a fail-open: a clause-initial capitalized common word before "API" ("Payment API", "Search API") reads as a service name; a long clause pairs a verb with a distant noun; and a CommonMark inline code span that wraps a newline is matched within-line only. Codex judged these acceptable because the COVERAGE.md declaration is a cheap override. One documented limitation remains out of scope: "Integrate Stripe, and authenticate requests with its API" (a coordinate finite clause whose noun refers back by pronoun) needs coreference resolution, beyond a lexical detector. Verified: 379/379 affected + command-router tests pass (+14 new regression tests covering every round-3 finding, both directions); tsc + eslint (no-adhoc-markdown-parsing) + lint:regression-names clean. Two fail-open cases are now DOCUMENTED limitations, pinned by tests so a future maintainer does not re-add the heuristics that caused the false positives above: a service named only in a clause separate from its API noun, and a bare external host that names no vocabulary word. Both are cheaply covered by the COVERAGE.md declaration and rare in real phase prose ("integrate the X API"). Also fixed from the round-4 review: - Qualification now survives markdown emphasis ("The **internal** Payments API" stays clean) while still not crossing a sentence/clause boundary. - readPhaseScope fail-closes on a REAL read failure (EACCES/EIO) enumerating the phase directory or reading the roadmap fallback — not only per-plan-file failures; a missing directory/section remains a legitimate no-op. The declaration-override path surfaces scope_read_error so an incomplete-scope override stays visible. - SERVICE_SURFACE_API_RE length-bound comment no longer overclaims. Net: the detector is same-clause verb+noun + `<Service> API` surface, with path/code/inline masking and a fail-closed posture. refactor(#2246): centralize cross-platform path-separator handling (toPosixPath / toNativePath / posixNormalize) (#2247)
Replace every open-coded separator translation across the installer/hooks
source with named, tested seams in shell-command-projection.cts (the platform
seam), removing all hardcoded `/`+`\` from path handling:
- toPosixPath(p) — this machine's native path → POSIX (running-OS relative;
for local filesystem paths).
- toNativePath(p) — POSIX → native (collapses the win32 `/\//g,'\\'` ternary).
- posixNormalize(p)— unconditional `\`→`/`, OS-independent; for emitting paths
to a POSIX/bash TARGET (which may differ from the running
OS) and for parsing mixed-separator input.
core-utils.toPosixPath now delegates to the seam, so its 20+ existing consumers
resolve to one implementation; no duplicate helper.
- ~47 sites across runtime-hooks-surface, runtime-artifact-conversion,
runtime-artifact-install-plan, drift, init, worktree-safety,
installer-migrations, installer-migration-authoring, install-engine, surface,
verify, runtime-artifact-layout, schema-detect, check-command-router.
- Closes the latent POSIX-literal-backslash corruption class (the regex form
corrupts a POSIX path containing a literal backslash; split(path.sep) does not).
- New unit + fast-check property tests for all three helpers.

Final convergence review found the shared <tag> seam introduced 3 behavior regressions; fixed all + locked with tests: - #557 REGRESSION: stripTaggedBlocks's attribute-tolerance stripped `<details open>` (the ACTIVE-milestone marker) that the old `<details>`-only regex preserved. The seam now takes `allowAttributes` (default false) — details/decisions strip is attr-INTOLERANT (preserves `<details open>`); only `<task type="…">` opts in. Regression test added to roadmap-parser + markdown-sectionizer suites. - verify.cts actionZones (negative-grep-echo security scan): reverted to a bounded to-first-close scan `<action>([\s\S]{0,20000}?)</action>` so a grep-echo trick can't hide behind an unterminated inner <action> (the seam's stop-at-next-open would drop it). ReDoS-safe via the cap. - check-command-router HTML-comment strip: `(?:-->|$)` fallback wiped to EOF (fail-closed spurious gate block) — replaced with stop-at-next-open so an unclosed `<!--` leaves downstream tags intact. - Updated the extractTaggedBlocks nested-tag tests to the new (stop-at-next-open) behavior: `<x><x>inner</x></x>` -> ['inner']. A convergence audit showed the `<tag>[\s\S]*?</tag>` lazy-scan ReDoS was pervasive (a dozen+ bespoke copies across roadmap-parser/check-command-router/verify), each a distinct quadratic vector on a large document with unclosed tags. Rather than whack-a-mole, single-source them (maintainer-directed): - markdown-sectionizer: extractTaggedBlocks now shares one ReDoS-safe `taggedBlockPattern` (stop-at-next-open, bounded optional attributes) and gains a `stripTaggedBlocks` companion for block removal. - roadmap-parser: 3 `<details>` strips -> stripTaggedBlocks (behavior-identical — no <details> here carries attributes). - verify: actionZones + both <task> loops + their nested <name>/<files>/gate/req extractions -> extractTaggedBlocks (behavior byte-equivalent, verified). - check-command-router: the objective|tasks?|action alternation hardened in place (distinct multi-tag shape); HTML-comment strip gains a `$` fallback. Every vector now linear (<3ms on 1.5MB adversarial); real content unchanged (end-to-end verify/roadmap resolution + task extraction confirmed). Replace plan-phase.md §5.6 (a 6-branch inline UI gate) with a capability-driven
loop.render-hooks plan:pre dispatch — the FIRST gate dispatch in any workflow.
A step (ui-phase, when:workflow.ui_phase) + a new blocking gate
(when:workflow.ui_safety_gate). New ui.plan-gate check verb returns
{frontend, hasUiSpec, block}; the dispatch runs it unconditionally then fires
the active step (pipeline) or halts on the active blocking gate (manual). The
gate-handling (run check.query; halt if blocking+block) is the reusable
phase-6 template for blocking-gate cutovers.
Config semantics fixed per #1022 + maintainer call: ui_phase gates plan-time
UI-SPEC generation, ui_safety_gate gates the planning block. Common case + all
ui_phase=false cases are equivalence-preserving; the one intended change is
{ui_phase:true, ui_safety_gate:false} now auto-generating in pipelines.
Review found it broken twice (non-generic dispatch, phase-lookup divergence,
then the step-only check nested in a gate loop) — fixed; final Codex pass
verified all 8 (ui_phase,ui_safety_gate)x{pipeline,manual} cases correct.
gsd-ui-phase skill + autonomous §3a.5 untouched (§3a.5 deferred).
getRoadmapPhaseWithFallback mirrors cmdRoadmapGetPhase for lookup parity.

Add `windowsHide: true` to every external-binary spawn in the runtime source: - hooks/gsd-context-monitor.js (record-session spawn) - hooks/gsd-worktree-path-guard.js (SPAWNOPT) - hooks/gsd-workflow-guard.js (git branch --show-current) - src/shell-command-projection.cts (execGit / execNpm / execTool) - src/check-command-router.cts (git log execFileSync) - src/roadmap-upgrade.cts (git status/rev-parse/reset/clean execSync) gsd-check-update.js already had it (the precedent). probeTty's tty call is POSIX-only and intentionally untouched.
