a9a7a328e6 refactor: hard-fork GSD -> MSD (Make Software Done)
Mechanical rename produced by scripts/msd-rename.cjs: gsd/Gsd/GSD -> msd/Msd/MSD
across contents and paths, upstream package/repo coordinates -> @golem15/msd-core
and golem15com/msd-core. Deep links into upstream history, sibling upstream
packages, the GSD-2 import feature, CHANGELOG.md and .changeset/ are kept as-is.

Hand edits on top: MSD block-letter banner and logos, LICENSE copyright line,
package/plugin identity, regenerated lockfile, install-tree fixtures, derived
registries and benchmark baseline; migration checksum baseline re-locked
(MSD keeps its own install state, so no install had applied the old sums);
sort-order and regex-escaped expectations in tests adjusted.

374300da17 fix(#4652): confine every boundary that joins argv to a managed root
Phase 2 of epic #4636, absorbing #4327 and #4354. Implements ADR-4650
decision 3: containment is a boundary concern — the predicate runs where
external input enters, not at whichever interior call site remembered.

Four boundaries now validate against their managed root and reject with a
USAGE-shaped error before touching the filesystem:

  todo complete <name>                 -> todosDir(cwd)
  check predicate --phase-dir <dir>    -> projectDir
  check decision-coverage-plan <dir>   -> projectDir   (via resolvePath)
  check gap-analysis.plan-post <dir>   -> projectDir

#4327 understated its own severity. It reports that a traversal name
"resolves outside the todos root", which reads as an information leak.
Measured, it was destructive: the command exited 0, MOVED the outside file
into completed/, and unlinked the original. cmdTodoComplete ends in
fs.unlinkSync(sourcePath), so an unconfined name consumed across the
boundary rather than merely reading across it. Validation now precedes every
fs call — existsSync, readFileSync, ensureDir, writeSync, unlinkSync — and
both halves of the move are confined, so neither source nor destination can
land outside the root. --dry-run is rejected on the same terms; a preview
must not leak a resolved outside path either.

#4354 reproduces exactly: a BLOCKING gate returned block:false sourced
entirely from a SECURITY.md in a caller-chosen directory outside the project.

THE HARDER HALF, found by the isolated adversarial review of the first
attempt: validating a path and then using a DIFFERENT one closes nothing.
The first fix validated `--phase-dir` joined against `--cwd`, then passed the
RAW unjoined value into the predicate context. gate-predicate-evaluator uses
it as-is and findPhaseArtifact resolves a relative path against the REAL
process cwd — so validation and the read used two different roots whenever
process.cwd() differed from --cwd. Reproduced: running from a directory
holding a plan with `secret_field: LEAKED_VALUE`, a predicate declared
against an empty --cwd project exited 0 and returned "actual":"LEAKED_VALUE".

The rule now applied at all three router sites: **use the validated resolved
path, never the raw input.** Independently re-verified after the fix — the
lookup resolves in the --cwd project and no value leaks.

gate-predicate-evaluator.cts is untouched and still imports no fs. Confining
in the router is what keeps that pure-leaf contract intact AND covers
${PHASE_DIR} interpolation into command-exit-zero, which an evaluator-local
fix would have missed entirely.

Also fixed, same review: `todo complete .` and `..` passed containment
(they resolve to the pending dir, which IS inside the root) and then threw an
uncaught EISDIR with an absolute-path stack trace. Now a clean USAGE
rejection naming the real reason — "todo name is not a file" — rather than
borrowing the escape message, which would have stated something false.

Ripples discharged BEFORE the verification checkpoint rather than after, per
the Phase 1 retrospective: docs/reference/gate-predicates.md and
docs/CLI-TOOLS.md document the new constraints, CONTEXT.md records why
containment lives at the router rather than the evaluator, the changeset is
written, and the install-tree goldens were regenerated to confirm unchanged
(no new shipped file) rather than assumed.

3925839f2a test(#4652): failing-first coverage for the four unconfined boundaries
Phase 2 of epic #4636, absorbing #4327 and #4354. Tests only; no fix. These
MUST fail.

Four CLI boundaries join externally-supplied input to a managed root with no
containment validation. Each was driven through the real CLI and confirmed
unconfined before the assertions were written:

  todo complete <name>                     src/commands.cts cmdTodoComplete
  check predicate --phase-dir <dir>        check-command-router cmdCheckPredicate
  check decision-coverage-plan <dir>       check-command-router resolvePath
  check gap-analysis.plan-post <dir>       check-command-router

Boundary 1 is worse than the issue describes. #4327 reports that a traversal
name "resolves outside the todos root", which reads as an information leak.
Measured, it is destructive: `todo complete ../../../../b1out/leak.md` exited
0, MOVED the outside file into completed/, and unlinked the original. The file
was gone. cmdTodoComplete ends in fs.unlinkSync(sourcePath), so an unconfined
name does not merely read across the boundary, it consumes across it.

Boundary 2 reproduces #4354 exactly: a BLOCKING gate returned
{"block":false,"details":{"match":true}} sourced entirely from a SECURITY.md
in a directory the caller chose, outside the project.

Boundaries 3 and 4 are not named in the epic. Both accepted an outside phase
dir and exited 0.

Rows that exist because they are the ones nobody enumerates:

- ORDERING. A real file is created outside the todos root, then the traversal
  name targeting it is asserted rejected AND the outside file asserted still
  present and unmoved. #4327 notes the existence check and the move target
  BOTH follow the unvalidated join, so a rejection that lands after the read
  has already leaked — and, per the finding above, after the unlink has
  already destroyed.
- `a/../../b.md` — looks balanced, resolves outside.
- --dry-run must reject too; a preview must not leak a resolved outside path.
- ${PHASE_DIR} interpolation into a command-exit-zero predicate is the SECOND
  predicate kind, which a fix inside gate-predicate-evaluator.cts would miss.
- An absolute path INSIDE the project must still be accepted at every
  boundary — absolute is not a synonym for escaping.

Cross-boundary rows loop over one shared list of escaping inputs and assert
all four reject with the same shape, so four sites adopting one predicate
cannot drift into four rejection contracts.

Property tests cover BOTH directions — outside is always rejected, inside is
always accepted. A property asserting only rejection is satisfied by a
predicate that rejects everything, which is the degenerate-implementation trap
found in Phase 1's review. Both are seeded.

Regressions fold into the owning module suites rather than a new
tests/fix-NNNN-*.test.cjs, per scripts/lint-regression-test-names.cjs.

Batch 7 of 17 in the ad hoc timeout literal migration (epic #4445).
Replaces every bare numeric timeout/timeoutMs object-literal property in
tests/loop-render-hooks.test.cjs, tests/loop-walk.qa.test.cjs,
tests/loop-hooks-empty-points-e2e.test.cjs, tests/loop-hooks-ship-pre-e2e.test.cjs,
tests/loop-hooks-verify-post-e2e.test.cjs, tests/check-gap-analysis-plan-post-e2e.test.cjs,
tests/check-tdd-review-checkpoint-e2e.test.cjs, tests/execute-wave-post-gate-pipeline-e2e.test.cjs,
tests/plan-pre-hook-e2e.test.cjs, and tests/qa/tdd-walk.cjs with a named
constant, per eslint-rules/no-adhoc-timeout-literal.cjs. Removes the 10
files from the rule's allowlist.

Promotes a new shared class norm to tests/helpers/timeouts.cjs,
LOOP_HOOK_POINT_CLI_TIMEOUT_MS: 7 files independently arrived at the same
value for a single gsd-tools.cjs CLI subcommand invocation with no
confirmed subprocess fan-out. Reuses the existing PROBE_TIMEOUT_MS for
loop-render-hooks.test.cjs's 11 sites (same class, exact value match).
Adds 4 file-local constants for values that share a class with the new
norm or an existing one but diverge in pre-existing value, or that
numerically coincide with an unrelated existing constant without
matching its actual operation.

Isolated Standards-axis review caught that the new shared constant's doc
comment exhaustively enumerated 3 verb families while a 7th genuine site
(an `init new-project` invocation) also correctly belonged to the class;
fixed by rewording the comment to state the class definition (call
shape) first and list all 4 representative verbs, explicitly
illustrative rather than exhaustive. No value changed, no site's
classification changed.

No src/bin file touched, no numeric value changed anywhere.

Red phase: 4 of 10 fail against current source (#2316-1 ghost-ID warning,
-3 requirements_updated honesty, -4a v1-heading suppression, -6b all-orphan gap
rows). The other 6 are controls/boundaries that must keep passing — including the
#1159 deferred-heading guard and the literal "TBD" placeholder boundary.

phase complete parses a phase's `**Requirements**:` line from ROADMAP.md and
reconciles it into REQUIREMENTS.md. When a cited ID was registered nowhere, every
branch degraded to a no-op and the report was indistinguishable from a run that
applied every update: `requirements_updated: true, warnings: [], has_warnings:
false`, file byte-for-byte unchanged.

Four defects on that path, all long-standing (traced to 2bc295b32d, v1.2.0):

1. The only cross-check compared REQUIREMENTS.md's own body against its own
   Traceability table, so an ID cited by ROADMAP but defined in neither was
   invisible to it. `citedReqIds` is now hoisted out of the `if (reqMatch)` block
   and cross-checked; ghost IDs raise a warning through the existing
   warnings/has_warnings surface that execute-phase.md already prints.
2. `if (reqUpdate.ok)` had no `else`, so a Traceability-row write matching
   nothing was discarded silently. Misses are now recorded.
3. `requirements_updated` was set unconditionally inside `if (existsSync)`,
   reporting "the file was in the transaction" rather than "a write landed". It
   now reflects whether the content actually changed.
4. DEFERRED_HEADING_RE's bare `v\d+` alternative treated an active `## v1
   Requirements` heading as deferred, zeroing the body scan for that section.
   Dropped; `deferred|backlog|future` still match, so #1159's suite still holds.

Also fixes the same class in gap-checker: the `items.length === 0` early return
fired before ghost rows were folded in, so an all-unregistered phase reported
LESS than a partially-unregistered one. It is now gated on ghostReqIds too.

Follows the #2140 precedent in milestone.cts, which hardened this exact class for
`requirements mark-complete`; adapted to this function's single `warnings` array
rather than adding fields execute-phase.md never reads.

* fix(#2316): address review — milestone-aware deferred headings, probe-based ghost check

Independent review found the first pass green but wrong — the tests were built to
miss the shape that breaks.

BLOCKER: dropping the `v\d+` alternative from DEFERRED_HEADING_RE regressed #1159
against GSD's OWN shipped template. gsd-core/templates/requirements.md:35 ships
`## v2 Requirements` with the deferred-ness in the BODY PROSE ("Deferred to future
release"), not the heading — so `v\d+` was the only alternative matching it.
Deleting it made every scaffolded project emit a false traceability warning on
every phase complete, forever. The real problem is that `v\d+` matched BOTH the
active v1 and the deferred v2: deleting it swings from suppress-everything to
suppress-nothing. The heading check is now milestone-aware — a `## v<N>` heading
is deferred only when <N> is not the current milestone's major, resolved through
the existing state.cjs seam. Unresolvable milestone fails safe to the old
always-deferred behavior, since suppressing a warning beats spamming every project.

HIGH: the ghost check re-derived membership from two lossy index sets that
disagreed with the case-insensitive write paths, so it warned "not registered
anywhere" about an ID whose checkbox it had just ticked, and about a
case-mismatched ID whose write landed. It now probes the same two write surfaces
the writes use, mirroring milestone.cts's #2140 precedent (probe the write, don't
re-derive it) — which is what the original brief asked for and the first pass
didn't do.

HIGH: the cited-ID tokenizer split the rest of the line on whitespace, so every
trailing word became a "cited REQ-ID". The shipped roadmap.md template puts an
inline HTML comment on that exact line, so a fully correct run told users to
register `<!--` and `-->` as requirements. Cited IDs are now filtered to the
REQ-ID shape, matching what bodyReqIds/tableReqIds already require.

HIGH: gap-checker had the identical early-return defect 34 lines above the one
fixed — the could-not-parse branch gated ghost rows behind the same unguarded
items.length. One malformed <decisions> line made all ghost rows vanish.

Tests: the #2316-5 guard only covered `## Deferred`/`## Backlog`/`## Future` —
headings that trivially still match — omitting `## v2 Requirements`, the only
shape the change altered. Added the shipped-template fixture plus guards for each
finding above.

e3262d94d3 feat(capabilities): add claude-orchestration capability (Workflow backend) (#1143)
Default-off, BETA, claude-only capability adopting Claude Code's Workflow tool
(/effort ultracode, Agent SDK >= v0.3.149) as an optional parallel-execution
backend for the GSD loop. Restores the wave parallelism + plan-checker + verifier
that #853 forces inline on Claude Code, and folds gsd-ultraplan-phase under one
runtime gate.

- Pure fail-closed core (src/claude-orchestration.cts): detectWorkflowBackend
  (gate ladder: enabled -> Claude -> backend != inline -> nested+background host
  -> valid Agent SDK -> SDK >= floor; every miss degrades to inline) and
  emitWorkflowScript (waves -> parallel() barriers, plans -> gsd-executor +
  worktree, files_modified overlap -> separate stages, resumeFromRunId, budget).
  All interpolated identifiers validated script-safe; briefs JSON-quoted.
- claude-orchestration command family (gsd-tools claude-orchestration
  detect-backend|emit-workflow) for orchestrator invocation.
- Two gated loop contributions at wired points (execute:wave:post, plan:post);
  federated config keys (enabled/execution_backend/min_agent_sdk_version).
- ADR-1143 implementation amendment; CONTEXT.md glossary entry; explanation doc.

On any runtime lacking the Workflow tool, behaviour is byte-identical to today.

The async external-job consumer half (#1165) shipped long ago: the core loop
reads .planning/async-jobs/<job>.json manifests and treats a non-terminal one
as the legal external_job_waiting half-state. The PRODUCER half (#1164) was
the remaining unimplemented piece of #1105.

This adds the producer as a default-off capability:

- capabilities/external-job/ — capability.json (execute:wave:post -> executor,
  plan:post -> planner contributions, external_job.* config keys, default-off)
  + fragments teaching runtime-budget classification and externalization.
- src/external-job.cts -> gsd-core/bin/lib/external-job.cjs — pure producer
  module: SLURM state -> manifest-status map (no guessing), manifest
  build/validate (versioned stability contract), sbatch/squeue/sacct parsers,
  and a fail-closed manifest writer (refuses a second non-terminal job for a
  plan_id already in flight; refuses to clobber a malformed manifest). fs/clock
  seams for deterministic tests.
- scripts/slurm-adapter.cjs — operator CLI (submit/poll/show) wrapping bounded
  sbatch/squeue/sacct subprocesses; surfaces manifest commands for confirmation
  and never auto-runs them (trust boundary).
- tests/external-job.test.cjs — 23 behavioral + fast-check property tests.
- docs/reference/long-running-operations.md + docs/how-to/async-external-jobs.md.
- CONTEXT.md glossary entry for the External-job Capability.
- Regenerated capability-registry.cjs; pruned the now-stale test-file-count
  allowlist entry (external-job is at the 2-file cap).

gsd-test caught that adding the external-job capability requires its
dependent artifacts regenerated and its registry-shape drift absorbed:

- sync-manifest-versions: stamp 1.7.0-rc.2 into capability.json (was 1.0.0).
- gen-capability-matrix --write: regenerate docs/reference/capability-matrix.md.
- gen-inventory-manifest --write: regenerate docs/INVENTORY-MANIFEST.json.
- check-gap-analysis-plan-post-e2e: plan:post now has 1 contribution
  (external-job planner fragment) instead of 0.
- execute-wave-post-gate-pipeline-e2e: execute:wave:post now has 2
  contributions (mempalace + external-job) instead of 1.

* fix(#1105): regenerate capability-registry after version stamp

sync-manifest-versions re-stamped external-job/capability.json from
1.0.0 to 1.7.0-rc.2 after the last registry regeneration, leaving the
committed capability-registry.cjs stale (CI gen-capability-registry
--check failed). gsd-test masked this because its setup runs the full
'npm run build' (which regenerates the registry); CI's 'npm test'
pretest only runs build:lib.

a375c4b354 feat(#956): add MemPalace memory capability (ADR-857 feature plug-in) (#1201)
* feat(#956): add MemPalace memory capability (ADR-857 feature plug-in)

Adds an opt-in, default-resilient ADR-857 feature capability that wires
MemPalace (local-first memory: MCP server + CLI) into the GSD loop:
deliberate recall before discuss/plan and verbatim + temporal-KG capture
at phase boundaries. Three memory modes (augment default; kg_backend and
replace forward-declared). Master gate mempalace.enabled (default off);
every hook onError:skip, zero gates; absent/disabled MemPalace => loop
unchanged. Transport is rendered-markdown only — MemPalace runs
out-of-process, no third-party code in gsd-core (ADR-857 §7).

Capability: capabilities/mempalace/ (manifest + 2 fragments), skills
commands/gsd/mempalace-{recall,capture}.md, agent
agents/gsd-mempalace-curator.md. Registration: ns-context router,
utility cluster, KNOWN_SKILLS, help full.md, model-catalog, copilot
install list, size baselines; regenerated capability-registry +
inventory manifest. ship:post wired into ship.md (wire-on-demand).

HELD on #1196: this capability also declares hooks at discuss:pre and
discuss:post, which are structurally un-wireable until the host-loop
conformance model covers the discuss phase (discuss-phase.md is not in
HOST_LOOP_FILES). The phase6-capstone-conformance gate therefore fails
on exactly those two orphaned points by design — see #1196. Once #1196
lands, rebase onto next and the gate goes green with no further change.

b10e56818b feat(#1169): complete ADR-857 phase 6 — migrate features to Capabilities, revive dead gates, harden conformance gate (#1183)
* test(#1168): make phase-6 gate un-gameable — reject empty stubs + require loop shrink

The migration assertion previously checked only role==feature, so a registration-only stub (empty hooks, logic left inline) would turn the gate green while phase 6 stayed incomplete — the exact false-completion pattern this gate exists to prevent. Strengthen it: each ADR-named feature must OWN its behavior (>=1 hook, or a command family); and plan-phase.md/execute-phase.md must shrink strictly below their frozen pre-phase-6 sizes (94519/93166 LF bytes), which also defeats double-run gaming (declare a hook but keep the inline block -> file does not shrink -> red).

Gate now 5 pass / 4 fail (orphaned execute:wave:post, empty/unregistered features, config-key leaks, no shrink). Green is now reachable only by REAL migration. Refs #1168, #1169.

- capabilities/gap-analysis/capability.json: role:feature, plan:post gate (when=workflow.post_planning_gaps, blocking:false advisory), OWNS workflow.post_planning_gaps (federated out of central schema). - plan-phase.md: inline config-get + gsd_run gap-analysis block replaced with a plan:post render-hooks call site dispatching the gate; file shrinks 94519->93279. - src/check-command-router.cts: cmdGapAnalysisPlanPost runs the real gap analysis via gap-checker. - post_planning_gaps removed from central manifest; resolves via federated config (default true preserved). - tests/post-planning-gaps-2493: re-pointed to assert capability ownership.

Verified: gate 5 pass / 4 fail (gap-analysis cleared from migration, plan:post-orphan, config-leak, and plan-phase shrink checks); loadConfig still returns post_planning_gaps=true; check command runs real analysis; 392/392 in the config/registry/federation/router net. Refs #1169.

The migration prose had downgraded this to a 'strong advisory recommendation' — silent loss of the blocking escalation. Restore it: the tdd-gate dispatch now refuses to mark the phase complete (Phase blocked message) under MVP+TDD when GATE_RESULT.block is true; advisory otherwise.

Also strengthen tests/execute-mvp-tdd-gate.test.cjs: hasBlockingEscalation previously matched any line with 'blocking'+'mvp+tdd' (so 'advisory (blocking: false) ... under MVP+TDD' was a false green); now it requires the real refusal semantics ('refuse to mark the phase complete' / 'phase blocked'). Caught by 2nd adversarial review pass.

  pre-phase-6 ceiling while keeping every onError/blocking nuance).
- capabilities/tdd inline fragment hardcoded `@~/.claude/gsd-core/references/tdd.md`,
  baked verbatim into the committed capability-registry.cjs and leaked the
  install path on 11 non-Claude runtimes (registry .cjs is copied, not
  path-converted). Made the fragment path-free; regenerated the registry. The
  phase-6 conformance gate now guards this (no ~/.claude install path in any
  capability source or the generated registry).
- plan-phase.md: removed a §5.7 stub re-added in error and routed Branch 2 to
  step 6 (schema-gate is a plan:pre capability, §5.7 is gone).

Stale workflow-contract tests re-pointed to the capability dispatch they now
must assert (behavior verified preserved in source first, assertions kept
equal-or-stronger): bug-621 + bug-2851 (gap-analysis via gsd_run render-hooks
plan:post + registry binding), feat-2527 (tdd_mode federated out of central),
phase6-planning + plan-phase-ui-redirect (§5.6 bounded by ## 6.),
plan-phase-drift-guard (intel when:intel.enabled skip branch).

profile-pipeline-command-router.cjs un-ignored from eslint (hand-written, no
TS source) + stale disable comments removed. Size baseline regenerated.

Verified: full suite 15140 tests / 0 fail; lint 0 errors; conformance gate green

Adversarial-review (Codex) findings fixed:
- HIGH: execute-phase.md TDD_MODE used `jq ... || echo false`, silently
  disabling the MVP+TDD blocking gate on jq-less runtimes. Reverted to the
  `node -e` form (node is guaranteed; matches the file's other node-e usages) so
  a missing optional tool can no longer fail-open a blocking safety path.
- MEDIUM: federated-config-key-removal orphan-key test was vacuous (it skipped
  the orphan assertion). Now asserts the removed capability's key is genuinely
  not surfaced/validated after uninstall.
- LOW: phase-6 conformance leak regex broadened to catch absolute-home and
  Windows-backslash `.claude/(gsd-core|commands|agents|hooks)` paths, not only
  `~`/`$HOME` forward-slash forms.
- LOW: bug-2851 plan:post dispatch assertion now requires `--raw` (matched its
  stated contract).
- nit: plan-pre intel-step test duplicate assertion replaced with a distinct
  structured-output check.

compares results by title and reported 29+29 false "only in Mac / only in

Move the golden path out of the title and into the assertion message (still
shown on failure); titles are now byte-identical across platforms so the
cross-platform comparator matches them. No assertion logic or golden values

blocking safety gate; `config-get workflow.tdd_mode` is forbidden by the
conformance leak gate (tdd_mode is capability-owned).

Correct fix (what Codex recommended): a gsd_run-native boolean. Add an
`--active-cap <capId>` flag to `loop render-hooks <point>` that resolves hooks
the normal way and prints exactly `true`/`false` for whether a capId is active
— scanner-safe (canonical launcher, no inline code), node-reliable (no optional
jq to fail-open), and leak-free (render-hooks resolution, not config-get).
execute-phase.md:181 now `TDD_MODE=$(gsd_run loop render-hooks execute:post
--active-cap tdd)`. +5 behavioral tests for the flag.

Verified: prompt-injection-scan --diff origin/next → 0 findings; conformance
gate 13/13 (execute-phase.md 92934 < 93166); execute-mvp-tdd + tdd-mode +
