refactor: hard-fork GSD -> MSD (Make Software Done)
Mechanical rename produced by scripts/msd-rename.cjs: gsd/Gsd/GSD -> msd/Msd/MSD across contents and paths, upstream package/repo coordinates -> @golem15/msd-core and golem15com/msd-core. Deep links into upstream history, sibling upstream packages, the GSD-2 import feature, CHANGELOG.md and .changeset/ are kept as-is. Hand edits on top: MSD block-letter banner and logos, LICENSE copyright line, package/plugin identity, regenerated lockfile, install-tree fixtures, derived registries and benchmark baseline; migration checksum baseline re-locked (MSD keeps its own install state, so no install had applied the old sums); sort-order and regex-escaped expectations in tests adjusted. |

enh(#2876): retire the dead and pass-through exports from bin/install.js (#3615)
* enh(#2876): retire the dead and pass-through exports from bin/install.js The installer exported 197 names and had zero production consumers - every non-test require of it repo-wide sits inside a comment. Its interface was shaped by test access, not by callers. Removes 9 dead exports and 61 pass-throughs, repointing their tests onto the extracted modules' own interfaces. 197 down to 127. Every count in the issue was wrong: 197 exports not 188, 9 dead not 12, 61 pass-throughs not 49, 44 test files not 42 - and the audit itself then missed 7 more consumer files. restoreUserArtifacts was on the dead list but ceased to exist in phase 6, and two _GSD_EFFORT_MANIFEST_* names listed as dead are now genuinely asserted, so acting on that list would have deleted live exports. 7 of the 9 dead names collide with an independent declaration that install.js delegates TO. Each removal was justified by which declaration a reference resolves to, never by whether the name appears somewhere. Coverage parity was the gate rather than test greenness: per-file counts were captured before any edit and diffed after. 44 of 45 files are byte-identical; the single delta is one added assertion, not a loss. The sweep for scattered require sites found two forms static grep misses - require(VARIABLE) and multi-line require() - plus tests asserting that install.js re-exports the SAME object, which now assert retirement instead. The DEFECT.GENERATIVE-FIX single-owner guard from #1511 had been repointed from a reference-identity check to install.X === undefined. Those are not equivalent: the guard exists to catch a duplicate function body reintroduced into install.js, and the replacement passes cleanly if that duplicate is used internally and never exported. It now walks bin/install.js's real top-level bindings, so it catches a duplicate under either shape, exported or not - strictly stronger than the check it replaced. Proved by injecting a duplicate and watching it go red. That weakening survived the coverage-parity gate because the assertion count never moved. The gate compares counts, so an assertion that changes meaning rather than number is invisible to it. Removing the exports had orphaned their wrapper bodies: 14 dead wrappers, 9 consts and 9 destructure entries, several pre-existing and found by the same sweep. Dead code left in the file this phase exists to shrink. Three more comments claimed re-exports this phase removed, and tests were reading Cursor and Windsurf hook constants from install.js's local copy while calling functions from the hooks surface - equal today, with nothing holding them equal. The local consts now reference the owning module. fix: recover silently-excluded test dirs + test-architecture audit hardening (#1195)
* fix: recurse test discovery so subdir test suites actually run
scripts/run-tests.cjs discovered tests with a flat readdirSync(testDir),
silently excluding tests/observability/ (4 files), tests/dispatch/ (1) and
tests/installer-migrations/ (1) — 94 passing tests — from `npm test` and all
CI lanes. Walk the tree recursively (relative subpaths preserved), classify
suites by basename, and add a fail-on-zero-executed guard for suite/default
runs (escape hatch GSD_ALLOW_EMPTY_SUITE=1) while preserving the empty
--files/--files-from path the CI inert lane relies on.
Unit suite 735 -> 741 files; surfaces ADR-227's observability/dispatch seam.

Adversarial verification confirmed these 5 prove nothing — their coverage is
provided more strictly elsewhere:
- enh-2790 'has a name: field' spot-checks (command-contract enforces /^gsd[:-]/)
- command-routing-hub duplicate construct + duplicate ERROR_KINDS assertions
- no-cjs-sdk-handsync-tooling (guarded files that never existed on main; bug-190
covers the real retired SDK artifacts)
- runtime-artifact-layout cline edge case (subsumed by the explicit-global test
and bug-782-cline-skills-emission)

ADR-218 (reject leading-zero versions like 1.01.0; npm duplicate pre-check) had
zero tests — the logic lived only in release.yml bash. Add a test that extracts
the actual rejection regexes from the workflow and exercises them against a
boundary table (leading-zero/malformed rejected, valid accepted) plus structural
wiring assertions. Goes red if the regex is reverted to [0-9]+.

Per the ADR test audit, rewrite 27 weak test files (test-only, no source
changes) so each can go red for the defect it guards:
- kill pass-always assert.ok(true) placeholders (research-cli, worktree-baseref,
bug-260 security guard, eslint-rules x24, clusters '|| true')
- replace source-text grep with behavioral calls (install Kilo, sh-hook-paths,
plan-review-convergence) and add a repo-layout governance test
- de-flake real-clock/Math.random coupling (phase last_updated, bug-3707 mtime,
context-utilization property, feat-3594)
- fix independence/shared-state violations (bug-492 singleton, issue-844 tmpRoot,
core reapStaleTempFiles, active-workstream TTY, feat-488 GSD_HOME)
- strengthen property/shape-only tests (research-provider/store classification +
collision) and unconditional plugin.json schema validation (issue-766)
Verified: all 28 files run together 1220 pass / 0 fail / 1 skip.

* chore: add no-tautological-assert lint rule, error in test suite
New custom ESLint rule (eslint-rules/no-tautological-assert.cjs) bans asserts
that can never fail: assert(true)/assert.ok(<always-truthy literal>),
'cond || true' inside an assert, and equality asserts comparing two identical
literals. Wired as error on tests/**; full sweep confirmed zero existing
violations so the suite stays green. Prevents the placeholder-assert regressions
the audit redesigns just removed. RuleTester coverage added (6 valid, 8 invalid).
Note: no-only-tests was already enforced via eslint-plugin-no-only-tests, so no
duplicate rule was added.

ADR-456 requires any allow-test-rule exemption added after the ADR to carry a
tracking issue number, but nothing enforced it. New ratchet gate
(scripts/lint-allow-test-rule-refs.cjs, wired into lint:ci) fails when a NEW
allow-test-rule comment lacks a #NNN/URL reference; the 323 existing untracked
exemptions are grandfathered in an allowlist that ratchets down as they gain
refs. Red-green verified (novel untracked offender fails; compliant passes).

* refactor(#1059): phase 5f-1 — extract standalone hook-surface writers into a module Extract the structurally-isolated hook-surface writer functions (cline/cursor/ copilot/codex-hooks-json + buildHookCommand + atomicWriteFileSync + node/bash runner resolvers) out of bin/install.js into a new src/runtime-hooks-surface.cts module (-693 LOC from install.js). Behavior-preserving: install.js requires + re-exports the moved functions (module.exports surface preserved); no descriptor reads, no behavior change. Prerequisite for the descriptor-drive (5f-2), mirroring ADR-3660's artifactLayout extract→drive split. Review caught + fixed 3 coupling issues: (HIGH) the module's atomicWriteFileSync dropped the shared __atomicWrittenTmps temp-tracking → now ONE shared set (module owns it, install.js aliases it, both cleanups read it); (drift) buildHookCommand called resolveNodeRunner(opts) vs the original resolveNodeRunner() → reverted; two source-grep tests (workflow-guard, sh-hook-paths) that scanned install.js for the moved functions → made behavioral/non-vacuous; duplicate runner resolvers consolidated. Settings-json hook block (~648 LOC) deferred to 5f-1b; descriptor-drive to 5f-2. New-module checklist done. ~62 hook test files green; gsd-test 17592/0.
