* enhance(#4139): Phase 7 — the agent-skill seam picks the payload in code

ADR-4139 stream 2. The non-Claude `#2454` persona fallback in cmdAgentSkills
(src/init.cts) now selects between a canonical agents/<name>.md and a
token-minimized agents/<name>.compact.md sibling based on
workflow.compact_content, resolved in code (a real function call with a real
exit code) rather than a prose config-get gate — the same precedent stream 1's
spine/detail split established for a load-bearing seam, applied here because
this seam already runs through TypeScript instead of an eager @-include.

A missing compact sibling falls back to the canonical persona and discloses
the fallback in the served payload itself (a leading HTML-comment provenance
line), so the Done-when contract — compact when on, canonical when off, never
silent or empty — holds even for an agent nobody has compacted yet.

Authored a .compact.md sibling for all 35 shipped agents (agents/gsd-*.md),
each an independent, complete rewrite (not an extraction — nothing is "moved"
the way spine/detail moves text) that preserves frontmatter, every @-include,
every output-format contract, and every guardrail verbatim while cutting
restatement and verbose framing. Verified mechanically: every pair registers
(a canonical sibling exists), every compact file is strictly smaller, and the
full @-include set matches canonical's — including which references are
standalone eager-load lines versus inline prose mentions, since demoting one
to inline changes what the host actually substitutes.

Traced the install path before writing any code (.gsd/phase/.../40-design.md):
stageAgentsForRuntimeWithConverter glob-copies every agents/*.md file with no
stem filtering under the default full profile, so the new .compact.md files
install for free with zero installer changes — matching issue #4407's stated
scope. A tiered agent profile that doesn't stage a compact sibling degrades
through the same fallback-with-provenance path already required for an
unauthored one, so no installer change is needed there either.

Extends tests/helpers/compact-content-variant.cjs with an AGENTS_ROOT export
(deliberately not folded into DEFAULT_VARIANT_ROOTS, since agent variants are
reached by a generic code construction rather than a literal path in prose,
and checkReachability's markdown-search shape has nothing to find there).
Reachability is instead proven behaviorally: tests/agent-skills.test.cjs's new
"#4407 compact payload selection" describe block spawns gsd_run agent-skills
against real compact/canonical fixture pairs and asserts on the served
payload, which can only pass if the seam genuinely wires through.

Fixed a pre-existing test whose agents/*.md glob incidentally matched the new
.compact.md siblings (tests/agent-skills.test.cjs's Skill-frontmatter drift
guard) and added the 35 new agents/*.compact.md entries to docs/INVENTORY.md's
roster, both real, unrelated-to-content defects the new files' mere existence
surfaced.

Regenerated: install-tree fixtures (19 runtimes now ship 35 more agent files
under the full profile), INVENTORY-MANIFEST.json, and the variant-swap token
benchmark baseline (npm run benchmark:compact-content-variants --write).

Closes #4407.

source/degraded (added by the prior commit, comment left stale).

Security review and the Spec axis found no defects requiring a code change;
their non-blocking observations (a pre-existing, unmodified path-construction
pattern; the reasoned, documented substitution of a behavioral test for the
literal reachability check) are recorded in
.gsd/phase/enhance-4407-agent-skill-seam/60-review.json.

* fix(#4407): repo-wide roster/cap fixes surfaced by shipping .compact.md agents

Root-caused via a real gsd-test run (93 failures) rather than guessing which
tests glob agents/ naively. Two classes of defect, both genuine:

1. Identity-roster confusion (11 files/areas): many tests and one production
   script derive "the set of GSD agents" from `readdirSync(agentsDir).filter(f
   => f.endsWith('.md'))`, which incidentally matched the new .compact.md
   variant siblings too — a compact file is a rendering of an EXISTING agent
   identity, not a new one. Fixed at the shared root
   (tests/helpers/agent-roster.cjs's listAgentFiles, which several tests
   already consolidated on) and at each independent glob that didn't use it:
   agent-size-budget.test.cjs (tier-cap lookup now strips the .compact suffix
   before checking XL/LARGE membership, so a compact file inherits its
   canonical sibling's tier instead of silently falling through to DEFAULT),
   agent-skills-bootstrap.test.cjs, check-contract-drift.test.cjs (the actual
   script, not just its test), codex-config.test.cjs (confirmed directly
   against generateCodexAgentToml that a compact role's derived sandbox_mode
   is byte-identical to its canonical sibling's before excluding it — not
   assumed), and copilot-install.test.cjs (two counts that legitimately DO
   need both files — an installed-file count and a full-conversion smoke test
   — fixed to expect 70, not stay pinned to 35).

   no-bare-gsd-tools-command-position.test.cjs needed the opposite kind of fix:
   two compact files reproduce descriptive prose already allowlisted at their
   canonical file's line number; added matching entries at the compact files'
   own line numbers rather than excluding them from the scan (a genuine bare
   gsd-tools command-position bug in a compact file would be as real a defect
   as in canonical).

2. A hard, non-ackable cap (found via emitted-attribution.test.cjs's real-tree
   run): six agents' compact renditions (gsd-debugger, gsd-executor,
   gsd-phase-researcher, gsd-plan-checker, gsd-planner, gsd-verifier) exceed
   the 32,768-byte NEW_FILE_CAP (ADR-1610) even after aggressive compaction —
   confirmed structural, not a compaction-quality gap: each is dominated by
   content this phase's own rules require verbatim (the ~2.6 KB gsd_run
   bootstrap preamble runtime-launcher-parity.test.cjs requires inlined in
   every agent that calls gsd_run, output-format contracts, guardrails).
   ADR-4139's prescribed remedy (spine + lazily-read parts) has no landing
   spot in cmdAgentSkills's single-file synchronous read. Removed these 6
   compact files rather than ship an over-cap file or invent a multi-part
   read mechanism out of scope for this phase; recorded by name with the
   reason in .gsd/phase/enhance-4407-agent-skill-seam/40-design.md and
   50-test-matrix.md, per #4407's own "or explicitly recorded as not worth
   covering" allowance. Their canonical personas are served correctly today
   via the fallback-with-disclosed-provenance path this phase's own Done-when
   #2 already requires — 29 of 35 agents now have a compact variant.

Also fixes an unrelated, genuinely pre-existing defect this gsd-test run
surfaced: gsd-core/workflows/execute-plan.md sat 21 bytes over its own
DEFAULT-tier hard cap (40,960 bytes) at the branch point, before any change in
this PR touched it — confirmed via `git show <merge-base>:...execute-plan.md
| wc -c`. Per CLAUDE.md's no-deferral rule, fixed inline rather than filed:
two meaning-preserving trims in the <success_criteria> block (a repeated
parenthetical replaced with a same-exception reference; one redundant
qualifier dropped) bring it to 40,940 bytes.

Regenerated install-tree fixtures, INVENTORY-MANIFEST.json, and the variant
benchmark baseline to reflect the 6 removed files. Docs/INVENTORY.md's 6
now-orphaned roster rows removed alongside them.

Round 2 of the gsd-test-driven roster fixes: two checks assumed every agent
has a compact sibling (true for 29 of 35 after the NEW_FILE_CAP exception),
breaking once 6 stems legitimately have none.

- tests/agent-classification-parity.test.cjs: the INVENTORY.md parser was
  picking up the "### Compact Payload Variants" subsection's rows as
  phantom/uncounted entries in the primary/advanced/inventory-only
  classification this test validates — a compact row documents an existing
  agent's alternate rendition and never gets its own AGENTS.md heading, so it
  was never meant to participate in that classification. Excluded at the
  parser, not per-assertion.
- tests/copilot-install.test.cjs: the derived expected-file-list generator
  assumed every listAgentFiles() stem has a .compact.md source sibling;
  checks disk per stem now instead.

a27cb6b2fa enhance(#4139): Phase 6 — the lazily-read remainder and the artifact templates (#4540)
* enhance(#4406): the lazily-read remainder and the artifact templates

ADR-4139 Decision 3, Phase 6 of the #4139 Compact Content epic. Covers stream 1b
(gsd-core/workflows/<name>/{modes,steps,templates}/*.md) and stream 4
(gsd-core/templates/**) with a variant-swap mechanism, confirmed with the user:
two independent, complete files per covered path (canonical + .compact.md
sibling), with the gate picking which one gets Read at the call site. This is
a different shape from Phase 5's spine+detail partition, and is safe here
specifically because these files are already reached only by a runtime Read —
a missed Read already means zero overlay content today, with or without
workflow.compact_content, so selecting between two independently-complete
files introduces no new failure mode (documented in
gsd-core/references/compact-content-gate.md's new "Streams 1b and 4" section).

Disposition, after inspecting every candidate rather than trusting a byte-size
threshold (same rigor Phase 5 applied to review.md):

- Stream 1b: 1 of 78 files compacted (help/modes/full.md, a user-facing
  reference doc emitted verbatim, not orchestrator instruction). The other 9
  size-threshold candidates are dominated by fail-closed guards, exact CLI
  invocations, or output-format contracts (AskUserQuestion blocks) — recorded
  not-worth-compacting, same reasoning as Phase 5's review.md.
- Stream 4: a ground-truth reachability audit replaced the initial size-only
  candidate list. Two files (summary.md, user-setup.md) got compact variants;
  a third (spec.md) was drafted, then dropped after discovering its only two
  call sites are eager @-includes, not a runtime Read — stream-1 material
  hiding under gsd-core/templates/, not stream-4's actual mechanism. summary.md
  itself has 3 eager call sites and only 1 genuine runtime-Read call site
  (execute-plan.md); only that one was wired, so the compact variant's savings
  apply to the sequential single-plan execution path only.
- Discovered while auditing reachability: 12 gsd-core/templates/** files with
  zero references anywhere in workflow/agent/command prose, compiled source,
  or tests — dead scaffolding predating this phase. Deleted in this same PR
  per this repo's no-defer policy, after re-verifying against a computed
  path.join(...) pattern (not just a plain-string search) that nearly caused
  two genuinely load-bearing templates (user-profile.md, dev-preferences.md)
  to be misclassified as dead.

New checker (tests/helpers/compact-content-variant.cjs): registration,
reachability, protected-content-preserved, size-smaller — replacing Phase
3/5's disjointness/completeness checks, which assume a partition rather than
two deliberately-overlapping documents. The reachability check's own
"unprefixed match" guard had a real bug (rejected the repo's own
`~/.claude/gsd-core/...` convention), caught by running it against the
already-wired help/modes/full.compact.md pair rather than only synthetic
fixtures — fixed to anchor on the nearest `gsd-core` path segment instead.

Template consumer parity (tests/compact-content-template-variant-parity.test.cjs):
proves each compact variant's `## File Template` fenced block — the actual
output-format contract a generated SUMMARY.md/USER-SETUP.md is parsed
against — is byte-identical to the canonical file, then runs the one real
deterministic consumer (gsd-core/bin/lib/coverage.cjs's classifyContent,
backing `gsd-tools uat classify-coverage`) against content built from that
shared contract.

Added a sibling benchmark script (scripts/benchmark-compact-content-variants.cjs)
rather than extending the existing spine/detail one — different data shape,
and the existing script's own contract deliberately isolates it from a
test-only helper's shape changing.

Emitted-drift acknowledgement: not needed. Every changed/added path in this
diff is hand-authored and present in the diff itself, so diffEmitted's
attribution loop resolves `via` to the path's own source before reaching the
ack-lookup branch (same reasoning Phase 5 verified for its own diff).

- docs/CONFIGURATION.md and gsd-core/references/planning-config.md's
  workflow.compact_content entries described only the spine+detail mechanism
  (Phase 5) and were missing this phase's variant-swap mechanism and its
  benchmark:compact-content-variants script entirely — required since this
  PR's changeset is type Added (CLAUDE.md's "Missing Docs for Changesets"
  rule). Both now describe both mechanisms and which call sites are wired.
- Added the missing RED^-1/no-op fixture for checkProtectedContentPreserved:
  a canonical file with zero <!-- gsd:protected --> blocks must be a
  no-op, not a violation — the only branch of that function the existing
  fixtures didn't exercise.
- Collapsed findCompactFiles/findMarkdownFiles in
  tests/helpers/compact-content-variant.cjs into one findFilesWithSuffix
  helper — the two were identical recursive walks differing only in the
  extension predicate (minor Duplicated-Code finding).

gsd-test caught this, not static analysis: 10 real failures in
tests/copilot-install.test.cjs, tests/installer-migration-install.integration.test.cjs,
and tests/repo-layout.test.cjs — all downstream of bin/install.js's Copilot install
path, which does
fs.readFileSync(path.join(targetDir, 'gsd-core', 'templates', 'copilot-instructions.md'))
after copying gsd-core/templates/** into the target project, then merges it into both
.github/copilot-instructions.md and (local installs) AGENTS.md. The reachability audit
that flagged this file as dead checked src/*.cts and gsd-core/bin/*.cjs but never the
repo-root bin/install.js — a separately maintained installer bundle outside the
src/-to-gsd-core/bin/lib/ compiled-output convention. The fs.existsSync guard around
that read degrades to a silent skip rather than a crash when the template is missing,
which is why this surfaced only once the real E2E install test ran, not from any
static check.

Re-verified the remaining 11 deleted filenames against bin/install.js specifically
(plain substring and quoted-filename search) before trusting that list — all 11 have
zero hits there, confirmed dead by the same standard this one file failed.

Regenerated the installer emitted-tree goldens (tests/fixtures/install-tree/*.json) to
reflect the restored file, and corrected the "Removed" changeset (jolly-lynx-sprint.md)
and the phase design doc from 12 to 11 deleted files.

CI's own full-test matrix (not gsd-test's matrix, which does not run this
check) caught 4 more false-positive dead-template classifications via
tests/removed-but-needed-lint.test.cjs / scripts/lint-removed-but-needed.cjs
— a literal, word-boundary basename check across .github/workflows/,
gsd-core/, and docs/ (excluding docs/adr/** and docs/research/**) for every
file a PR deletes. It has no semantic awareness, so a deleted template's
basename colliding with something else entirely still fires:

- claude-md.md: gsd-core/templates/README.md had a stale table row claiming
  /gsd-profile reads this template to generate CLAUDE.md. Verified false (no
  code reads it anywhere, same search that already covered bin/install.js) —
  fixed the row to *(inline)*, matching every other command-generated
  artifact in that table. File stays deleted.
- codebase/testing.md: collided with docs/guides/testing.md, an illustrative
  example row in docs-update.md's sample output table (an unrelated real
  generated-docs path). Swapped the example topic to "contributing" — the
  row is illustrative, any topic works. File stays deleted.
- codebase/architecture.md, codebase/stack.md: collided with docs/reference/
  planning-artifacts.md's directory listing of a user's own generated
  .planning/codebase/architecture.md and stack.md output — the same
  semantic mismatch already investigated and dismissed as unrelated earlier
  in this phase's audit, now caught by a gate instead of judgment. That
  listing repeats across 5 locale copies of the doc.
- continue-here.md: collided with the real .continue-here.md pause-work
  artifact, referenced across 15+ locale and workflow files.

For the last two, the lint's own error message offers "restore the file or
update every consumer in the same commit." Rewording 15+ files across
languages I cannot verify translation quality for, to shave 2 already-tiny
templates that were merely presumed dead, is disproportionate to this PR's
actual scope — restored codebase/architecture.md, codebase/stack.md, and
continue-here.md instead, and corrected docs/ARCHITECTURE.md's Templates
section accordingly.

Final confirmed-dead set: claude-md.md, codebase/concerns.md,
codebase/conventions.md, codebase/integrations.md, codebase/structure.md,
codebase/testing.md, debug-subagent-prompt.md, discovery.md — 8 files, down
from the original 12. Verified locally: GSD_REMOVED_BUT_NEEDED_BASE=next
node scripts/lint-removed-but-needed.cjs now passes clean.

* fix(#4406): split codex-config.test.cjs to fix a genuine Windows CI timeout

Root cause of the `full test (windows-latest, 24, shard 2/3)` failure the
user asked to be actually fixed, not just re-run past: PR #4497 (landed
2026-09-07, one day before this PR's CI run) isolated
tests/codex-config.test.cjs into its own dedicated chunk because its
measured weight (17.87, ~45% of the post-cut Windows budget) made it unsafe
to share a chunk with any other file. That isolation was necessary but not
sufficient — even alone, with zero companion-file contention, the file's
real Windows execution time sits right at the 600s per-chunk ceiling. Two
independent CI runs on two unrelated PRs (this one and #4154) were both
killed within ~1.4s of the identical 600000ms mark — not random contention,
a deterministic near-miss the isolation fix couldn't address because it
never reduced the file's own cost, only removed the risk of a companion
file's cost stacking on top of it (which the PR #4497 comment explicitly
anticipated: "if a future profiling pass genuinely speeds up
codex-config.test.cjs itself, this isolation can be revisited").

The file itself explains why it's this heavy: 11,262 lines / 433 tests / 79
describe blocks, accumulated over dozens of bug-fix PRs (#2695, #2760,
#3245, #3285, #3346, #3426, #3427, #3562, #3566, #3582, #3808, and more),
several of which are explicitly documented as "folded" in from separate
files that were never actually split back out ("Verified non-duplicate
against both the pre-existing target and the other three folded sources").

Split into 4 files by top-level AST statement boundaries (never a naive
column-0 regex — an early attempt at that overcounted 79 apparent
"describe(" matches when only 21 are genuinely top-level; the rest are
nested inside a handful of large folded-in blocks, which a regex can't tell
apart from real top-level statements). Verified lossless twice: the split
script asserts byte-for-byte reconstruction of every source character, and
independently, total test()/describe() call counts match exactly between
the original file and the sum across all 4 new files (433/79 both sides).
Each new file carries the complete original shared header (imports/helpers)
for safety; per-file unused-import warnings from that duplication are
resolved via ESLint-precise alias renames (`{ foo: _foo }`, the standard
form for an intentionally-unused destructured binding — never a bare `{
_foo }`, which would destructure a different, nonexistent property).

No change needed to scripts/run-tests.cjs's ISOLATED_HEAVY_FILES or its
pinned test in tests/run-tests-harness.test.cjs: the file that keeps the
original name (tests/codex-config.test.cjs) is now only ~28% of the
original's size and safely isolated in its own chunk as before; the other
three new files re-enter normal weight-balanced packing, none individually
close to disproportionate. Confirmed no other file hardcodes the hardcoded
filename anywhere that would silently stop these tests from running (the
CI test-selection scripts determine scope algorithmically, not by literal
filename).

Adds an execCall kind alongside the existing regexTest detection --

file's real effective sites are covered by separate, already-cited

  exists to enforce, already cited in its file header) -- fixed to match.

- gsd-check-update-worker-platform-gate.test.cjs's deleted file-header
  marker was NOT dead weight: its codeOnly() helper wraps readFileSync
  and is called inline as an assert argument, a genuine source-grep
  pattern on real .cjs/.js source that the rule cannot currently see
  (helper-function indirection is a distinct blind spot from anything
  Phase 7/8 measured) -- CONTRIBUTING.md is explicit that "unverified"
  is not the same as "vestigial." Restored, site-scoped this time
  (directly above codeOnly(), not as an inert file-header comment) and
  cited (#3103, the issue the file's own docstring already references).

codex-config.test.cjs's two deletions and orphaned-hooks.test.cjs's /
settings-jsonc.test.cjs's deletions were independently re-verified and
stand: their flagged lines read generated .toml/.json OUTPUT, not
source, or have no residual pattern at all.

e9868a92ba fix(#1875): route installer settings/defaults writes through atomic, lock-guarded primitives (#3966)

writeSettings is the sole writer of settings.json/settings.local.json for
six runtimes and wrote them with a naked fs.writeFileSync. Hosts discard the
entire settings file on any parse failure, so a crash mid-write cost the user
every hook, permission, env var, and statusline they had — not just GSD's.

Route it through the atomicWriteFileSync (temp+rename) already used elsewhere
in the installer and already bound in this file.

withWriteFailure in the migration integration harness matched only the final
destination path, so an atomic write bypassed the injection entirely and turned
a rollback assertion into a vacuous pass. It now also matches the .tmp- sibling.

"preserve existing, don't touch" signal. The #338 migration coerced that null
to {} and wrote the result back, so a settings.local.json with one stray comma
lost all its non-GSD content on the next install.

The guard stands the whole migration down rather than just the local write:
skipping the merge while still stripping the shared file would destroy the GSD
entries outright instead of relocating them.

Aborting here reaches a pre-existing latent crash that the clobber had been
masking. Both are fixed, with their own regression test:

- the unparseable-settings guard returned bare `undefined` while all five
  sibling early exits return the full result shape, so installAllRuntimes'
  statusline lookup (results.find(r => r.runtime)) threw;
- handleStatusline dereferences result.settings, which is null on every early
  exit, so the call site now falls through to the banner branch.

Both crashes reproduce on unmodified next with a malformed settings.local.json
and no migration involved.

Every non-Claude install read-modify-writes ~/.gsd/defaults.json with no lock
and two separate naked whole-file writes. The file is read by every runtime and
project on the machine, so concurrent installs lost each other's key, and a
crash in either write window truncated it — silently, because the read path
swallows parse errors and treats a corrupt file as absent.

Take the existing acquireInstallMigrationLock around the read-modify-write and
apply both mutations in one atomicWriteFileSync. An install that changes nothing
no longer rewrites the file at all.

Existing semantics are unchanged: the explicit resolve_model_ids:true opt-in
(#1569) and an existing "omit" are preserved, non-canonical values still default
to "omit" (#1156), a pre-existing runtime string is preserved (#2395), the
malformed-non-object recovery (#1657) stands, and both console lines still print
when both keys change.

The #2834 structural test sliced a fixed 1200-character window from the function
source; the added lock comment pushed an asserted token past it. The window now
tracks the function body, so a comment or guard cannot red it spuriously.

15af0f5536 enhance(#3951): B6+B7 — widen two unreachable lint rules and make the guard ledger true (#3965)
* fix(#3951): two lint rules that could not reach the code they govern

B6 names two widenings. Measuring them first turned up a defect the criterion did
not know about, and refuted the reason it gave for one of them.

1. no-adhoc-markdown-parsing self-gates on its own filename.

   Lines 107-110 short-circuit create() to {} unless the path matches
   /(?:^|\/)src\/[^/]+\.cts$/. B6 says to widen the files: glob in
   eslint.config.mjs - but doing only that ships an INERT rule, because the gate
   still returns {} for every new path. Both halves have to change, and the gate
   is the load-bearing one.

   That same regex hides a live hole: [^/]+ is FLAT-ONLY, so it requires the file
   to sit directly in src/. The registered glob is src/**/*.cts, which includes
   subdirectories. 28 .cts files - health-diagnostic-rules/ (10),
   installer-migrations/ (11), observability/ (3), host-integration-adapters/ (2),
   vendor/ (2) - are inside the registered glob and silently skipped.

   Measured with the gate neutralized: 0 violations there today. The hole is
   hiding nothing right now, and is fixed anyway, because "no violations today" is
   not a property that keeps holding.

   The fix is not invented: require-subprocess-timeout.cjs:196 already carries the
   correct form of this guard, /(?:^|\/)src\/.*\.cts$/ with .*, one directory over.
   Checked the other 21 rules for the same bug - no-adhoc-regex-escape and
   no-private-binary-resolution short-circuit only to exempt their own seam file,
   which is the right shape, and no-crlf-fragile-split has no filename gate at
   all. This bug is unique to the one rule.

2. no-adhoc-regex-escape could not see the shape that actually occurs.

   Line 396 gated the whole UNSAFE-NEW-REGEXP arm on arg.type === 'Identifier'.
   Every check below it - the _SOURCE provenance check, the
   isSoleReturnOfOwnParameter shape - lives inside that branch, so
   new RegExp(obj['key']) and new RegExp(cfg.pattern) were never examined at all.
   Runtime data arrives as a property access far more often than as a bare
   identifier, which is exactly why this rule never fired on the #3477 ReDoS.

   Widened to MemberExpression, measured by AST walk across all five registered
   blocks rather than by grep. 27 sites, zero TSAsExpression:

     18  safe new RegExp(X.source, flags)  -> exempted, keyed strictly on the
         PROPERTY being `source`, never on the object. Keying on the object would
         wave through X.anything and buy nothing. B6 estimated ~10; that was an
         undercount.
      3  _SOURCE-suffixed constants reached through a required module namespace
         (phaseId.BRACKET_PHASE_TOKEN_SOURCE) -> the same provenance-exempt class
         the rule already recognizes for bare identifiers, extended to reach them.
         Without this the widening produces 3 false flags.
      6  real findings -> marked, each a test extracting a pattern from a shipped
         file at test time, where the runtime contract IS the product.

   Deliberately the NARROW MemberExpression form. The rule's own
   isSoleReturnOfOwnParameter doc comment records that an earlier broad
   "any non-literal identifier" heuristic produced ~25 false positives and was
   rejected; a re-run of the census after this change flags exactly the 6 above
   and nothing else.

Verified by execution, not by reading: the gate now accepts src/<subdir>/x.cts,
still accepts flat src/x.cts, and still exempts paths outside src/ - each pinned
by a test proven to fail against the old regex. build:lib, lint and lint:ci all
exit 0.

The rule self-gates on filename AND is registered on one glob, so widening either
half alone is inert. Both move here: the gate now accepts tests/**/*.cjs and
scripts/**/*.cjs alongside src/**/*.cts, and eslint.config.mjs registers it on the
same two.

A test pins that the gate and the registration AGREE, in both directions. The
original defect was a gate narrower than its registration; the failure mode of
this fix is a gate wider than its registration. Both are silent, so the test
asserts the pair rather than either half.

80 violations across 43 files, all in tests/, zero in scripts/. 70 are routed
through the existing seams - scanFencedBlocks, collectSection, stripFencedCode,
tokenizeHeadings from markdown-sectionizer; splitTableRow, parseMarkdownTable,
findTableWithColumns from markdown-table. Headerless STATE.md tables use
splitTableRow per line, because parseMarkdownTable needs a real delimiter row.

10 are suppressed, 12.5%, well under the third that would have meant the rule is
mis-scoped for tests/ rather than the tests carrying debt. Each names its reason:
three regression guards (#3873 / bug-#21) are deliberately independent of the
generator's own fence handling, and routing them through the seam would have them
test the generator against itself; one is a negative-text probe that extracts
nothing; six are a shell-pipe-to-jq detector whose regex coincidentally matches the
table fingerprint and is not markdown parsing at all.

All ten sit in tests whose subject is .md content, which is normally a reason to
prefer the seam. The marker used is allow-adhoc-markdown, distinct from
no-source-grep's allow-test-rule, and lint:ci's lint-allow-test-rule-refs reports
the same 280/280 unverified count as before - checked rather than assumed, because
those two markers are easy to conflate.

The widening earned its keep immediately: it found a test that passed for the
wrong reason.

  tests/config-field-docs.test.cjs asserted notEqual(<cell>, '600') against the
  TYPE column instead of the DEFAULT column. notEqual('number', '600') is true
  forever, so the guard against workflow.subagent_timeout regressing to the old
  seconds default could never fire. docs/CONFIGURATION.md:434 is
  `| workflow.subagent_timeout | number | 300000 | ... |`, so the default is cell
  index 2; the assertion is now row-scoped through splitTableRow and reads 300000.

That is the argument for the widening in one case: the violation was invisible to
lint, the suite was green, and the assertion was vacuous. A rule that cannot reach
a file cannot tell you the file is lying.

Not fixed here, and recorded rather than assumed: #3426/#3239 are NOT reachable by
this widening. tests/package-legitimacy-gate.test.cjs yields zero violations even
with the gate bypassed - its hand-rolled scans are real, but built from line
filters and split('|') rather than the regex-literal fingerprints this rule
detects. They need new detectors. The epic assumed a wider glob would catch them.

build:lib, lint and lint:ci all exit 0; the post-fix census across tests/** and
scripts/** is 0 violations.

B7 asks that each closed child be driven fail-first with a behavioral identity
test at the CONSUMER's output. Four of eleven children had no test citing their
issue number. Auditing them by BEHAVIOR rather than by number-grep changed the
answer for three of the four.

#3364 and #2540 — traceability only. Both were implemented by #3941 and their
consumer-output tests exist and were shown failing-first; neither cited its
originating issue, so an audit that greps for the number reports them uncovered.
Tagged the specific asserting test in each file, following the citation form those
files already use.

#3372 — covered, but only at helper level, and the triage narrowed it. Of the four
commands the issue names, only estimate-cli's collectCalibrationSamples actually
enumerates phase dirs from disk; smart-entry, audit and roadmap-upgrade derive from
ROADMAP/body text and never reach the sentinel path, so they are benign by
construction and were left alone rather than "fixed" into churn. The existing #3882
rows asserted the helper's return value. Added a consumer-output test driving
`query estimate-calibrate` and asserting sample_count and the persisted document.
RED proof: reverted collectCalibrationSamples to a raw readdirSync and ran the real
CLI - sample_count 3, sentinel leaked; restored - sample_count 2.

#3356 — NOT covered, and BOTH halves of the defect were still live in source. The
issue is closed; the bug was not fixed. Fixed here rather than writing tests that
document a bug as correct.

  Defect 1, the contradicted row. quick.md:627 claimed
  `quick-tasks-append` performs "the equivalent write" to the Step 7c row. It did
  not: the `#` cell was a positional ordinal and `Directory` read `—`, because the
  route had no way to receive a quick id or task directory. Added OPTIONAL
  `--quick-id` / `--slug` / `--directory`. A caller with neither - fast.md, the
  original #2133 caller - omits them and gets the byte-identical prior row, so
  nothing existing changes. A caller that HAS a real id and directory now gets the
  canonical row quick.md:632 renders. The false-equivalence sentence itself is
  corrected rather than left to mislead the next reader.

  Defect 2, the forced re-derive. The route called readModifyWriteStateMd with no
  options, so a body-only append to the Quick Tasks table triggered a full
  re-derive of the disk-derived progress.* frontmatter. Every other body-only
  writer passes { resync: false } - src/state.cts's own docstring prescribes it -
  and this route was the lone outlier. RED proof: reverted the option, seeded a
  project with 2 real phase dirs and a curated total_phases of 25, ran
  quick-tasks-append; total_phases collapsed to 2. Restored; it stayed 25.

That second one is the shape this epic exists to close: a silent write that
replaces curated state with a re-derivation nobody asked for, exit 0 throughout.

build:lib, lint and lint:ci all exit 0.

The ADR gains a ledger amendment in its own correction style - the sixth wrong
premise it records, found the same way as the other five, by measuring before
building.

B6 says the net guard count must fall. It rose: 62 -> 69, +7, measured from the
epic's filing commit to origin/next. The attribution is the point, though. Five of
the seven came from PRs unrelated to this epic, one was added by a phase of it, and
the epic did retire something sub-file - #3884 removed a detector with an explicit
"net: -1 detector, 0 added" ledger. Every named casualty is load-bearing, two
already carry retractions in this same document, and a sweep of all 22 rules plus
every scripts/lint-* found no provably dead guard. There is no honest way to make
the count fall; forcing it would trade coverage for a number, which is the Goodhart
outcome Decision 6 exists to prevent.

The amendment also records that B6's own prescribed fix for one widening was inert.
no-adhoc-markdown-parsing self-gates on its filename, so widening only the files:
glob - which is what the criterion says to do - ships a rule that still returns {}
for every new path. And #3426/#3239 are not reachable by that widening at all;
their scans use line filters and split('|'), not the regex fingerprints the rule
detects. The roster row tracked them against the wrong mechanism.

Three roster rows updated from aspiration to fact: the two widenings are DONE with
their measured counts, and lint-phase-enumeration-drift is marked RETAINED rather
than "expected casualty - verify before retiring", because Phase 5 verified it and
kept it.

The rule Decision 6 should carry forward is stated plainly: a guard ledger is a
claim about COVERAGE, not about COUNT. "Net count must fall" is measurable and
wrong. "Every guard is reachable, and each retirement names what makes its defect
unrepresentable" is the property that was actually wanted.

CLI-TOOLS.md documents the optional --quick-id/--slug/--directory flags and says
plainly that omitting them keeps the pre-#3356 row byte-identical, plus that the
append no longer re-derives progress frontmatter.

New features fragment (id 3951); FEATURES.md regenerated rather than hand-edited.
Changeset is Changed, pr:0 pending backfill.

They are stale tests, not a regression: four rows assert that
no-adhoc-markdown-parsing is inert outside src/*.cts, which is exactly the
contract this deliverable changes.

Confirmed by reading rather than inferred from the names - the row at :1981 used
filename: 'tests/some.test.cjs' and filename: 'scripts/helper.cjs', the two roots
the rule now covers on purpose.

Worth recording WHY local gates missed this. npm run lint and lint:ci were green,
and the touched test files passed standalone. Lint only reports violations in real
files; these rows assert the rule's REACH using synthetic RuleTester filenames, so
nothing but the full suite could see them. Local green on a rule change says
nothing about the rule's own tests.

Each row is rewritten with BOTH halves rather than flipped from valid to invalid:

  - the same fingerprint under tests/ or scripts/ is now flagged, with the right
    messageId
  - the negative space is preserved - the same fingerprint under a path outside
    all three roots (gsd-core/bin/lib/foo.cjs) is still NOT flagged

The second half is the one that matters. Without it the rule has no boundary and
nothing would catch an over-wide gate later, which is the mirror image of the bug
this deliverable just fixed.

Each row is renamed to state the current contract; the old names said
"non-src/*.cts ... is not flagged" and would have been actively misleading once
the bodies changed.

Proven to test the widening rather than restate it: every flagged half was run
against HEAD~2's pre-widening rule and does NOT fire there, then against the
current rule and does. 12/12 on that probe; the full file is 178/178.

Swept for the same staleness elsewhere and found none.
require-subprocess-timeout's own "inert outside src/*.cts" row is untouched -
that rule's gate was not widened here - and no-adhoc-regex-escape's test file
already carries correctly-targeted rows.

gsd-core/workflows/quick.md is runtime-loaded emitted content, so correcting

construction. This is the acknowledgment, not a workaround - there is nothing
to regenerate.

The fragment names ONE path, which is the only one the guard reported. The four
spent acknowledgments it also listed (audit-uat, plan-phase, progress, review)
belong to other fragments whose ripple the base already absorbs; they are inert,
not failures, and are deliberately NOT copied here - naming paths I did not
change would make this record false in the other direction.

Byte figure corrected before committing: the guard reported 37220 -> 37584
(+364), but origin/next has since moved and quick.md is 37232 there now, so the
measured delta is +352. The reason text says so and names the base as a moving
figure rather than pinning a number that is already stale.

trailer carries no number, which is the better design - the figure was stale
twice in two attempts.

Emitted-Drift-Ack-Growth: quick.md — #3356/#3951 replaces a false claim with an accurate one. Line 627 said the `quick-tasks-append` shortcut "performs the equivalent write" to the Step 7c row rendered above it; it did not, and that was the documented half of #3356 — with no quick id or task directory the route emitted a positional ordinal in `#` and an em-dash in `Directory`, a visibly different row. The corrected sentence has to carry three facts the original elided: what the shortcut actually writes when it has neither input, that this is honest behavior for its real caller (`fast.md`, which has neither), and how a caller with both now gets the byte-identical canonical row via the new optional `--quick-id`/`--slug`/`--directory` flags. Prose is the product here — an executing agent reads this line to decide whether the shortcut is safe for its case, and a shorter correction would either drop the flags (leaving the reader unable to act on the fix) or drop the limitation (recreating the false claim in gentler words).

ADR-3473 §8.3 has four rungs; #3883/PR #3896 shipped the first. This pins the

Rung 2 — the install marker has four readers and resolveRuntime is not one.

  resolveRuntime resolves GSD_RUNTIME > config.runtime > 'claude' and reads no
  marker at all, while bin/install.js writes one (#2297) and FOUR hand-rolled
  readInstallRuntimeMarker copies exist: src/model-resolver.cts:65 (cached, with
  test seams), hooks/gsd-agent-isolation-guard.js:112, and TWICE in
  hooks/gsd-cursor-subagent-start.js at :346 and :355. Four copies of one rule.

  Fixtures and seam names mined from PR #3382 rather than re-derived; it
  implemented this rung and was closed "not on the merits".

Rung 3 — the sandbox map, and the fallback that was the real defect.

  Measured across all 35 files in agents/, deriving workspace-write iff tools:
  declares Write or Edit:

    - all 11 CODEX_AGENT_SANDBOX entries derive to their mapped value exactly,
      zero disagreements — the map carries nothing the contract does not
    - 24 roles fall through `|| 'read-only'`, of which 16 declare Write or Edit

  So the map is redundant and the silent fallback is the defect. The maintainer
  chose to derive but hold those 16 at read-only pending the question of whether
  Codex enforces sandbox_mode or merely advises; HALT.md records it.

  T20 asserts the emitted sandbox_mode PER ROLE against a captured baseline, not
  in aggregate — an aggregate passes while one role silently widens, which is
  the proxy-instead-of-identity shape this repo names. T24 and T25 fail on a
  stale hold, so the hold list cannot rot into the subset map being deleted.

Rung 4 — shortFormToId, recovered rather than invented.

  I nearly reported this as another wrong §8.3 claim: `git log -S shortFormToId`
  returns only documentation commits. That was the wrong instrument. Direct
  inspection of sdk/src/query/phase.ts at 11918dcc3^ shows five occurrences, and
  the tests match that code rather than a guess at its semantics — including
  first-write-wins on a duplicate short form.

  T43 asserts at the consumer's output: the emitted `waves` map from the real
  CLI, which pre-fix collapses to {"1":[...]} because every short-form edge is
  dropped. A unit assertion on resolveDependencyId would have passed throughout
  this defect's life.

Observed RED, this tree:
  rung 2   11/11 fail — no marker rung, no seams
  rung 3   T23,T24,T25,T26,T30 fail; T28 fails (validate agents passes a TOML
           whose sandbox_mode disagrees — it checks presence only)
  rung 4   T42,T44 fail; T43,T49 fail with waves collapsed to a single wave 1

Green and staying green: T20/T21/T22/T27 as captured baselines, #3885's
unresolvable-token warning and wave-verdict suppression, and #3785's
display-mapping passthrough. If the third tier over-reaches, those go red — that
is their job.

Disclosed weakness: T45 (a canonical id with no dash is not short-form indexed)
cannot be isolated behaviorally, because planMap always masks it. It is a
non-crash boundary pin, weaker than the other rows, and is recorded as such
rather than presented as equivalent.

ADR-3473 §8.3 has four rungs. #3883/PR #3896 shipped the first. These are the
other three.

Rung 2 — the install marker had four readers, and resolveRuntime was not one.

  resolveRuntime resolved GSD_RUNTIME > config.runtime > 'claude' and read no
  marker, while bin/install.js writes one (#2297) and four hand-rolled
  readInstallRuntimeMarker copies existed: src/model-resolver.cts (cached, with
  seams), hooks/gsd-agent-isolation-guard.js, and twice in
  hooks/gsd-cursor-subagent-start.js.

  model-resolver's was already the house idiom, so it was promoted rather than
  replaced: src/runtime-slash.cts now owns it, and model-resolver plus both
  hooks delegate. The hooks reach it through ensureRuntimeBuild(), the seam
  lint-hooks-runtime-build-seam enforces. No import cycle existed - checked
  both directions before moving anything.

  The marker is the THIRD rung: env > project config > marker > 'claude'.

  N1 was checked rather than assumed, and my first reading of it was wrong. A
  marker holding an unknown name comes back essentially verbatim, which looked
  like a validation gap. Measured against the env rung with the same inputs -
  including "../../etc/passwd" and "claude;rm -rf /" - the two are identical,
  because they share resolveRuntimeNameFromCandidates. N1 asks for exactly that,
  and it is met. The residual (the shared normalizer normalizes shape, it does
  not validate against the known-runtime set) is pre-existing on the env rung
  and plausibly deliberate, since a new runtime should not need a code change.
  The marker also does not widen the trust boundary in any real sense: it lives
  inside the install tree beside the code, so anyone who can write it can write
  runtime-slash.cjs itself.

Rung 3 — the map was redundant; the silent fallback was the defect.

  Measured across all 35 files in agents/, deriving workspace-write iff tools:
  declares Write or Edit: all 11 CODEX_AGENT_SANDBOX entries derive to their
  mapped value exactly, zero disagreements. The map carried nothing the contract
  did not already have, so it is DELETED rather than reconciled. What was
  actually broken is `|| 'read-only'`, which silently under-granted 24 of 35
  roles.

  16 of those 24 declare Write or Edit and would widen under derivation. Per the
  maintainer's decision (HALT.md), they are held at read-only pending the
  question of whether Codex enforces sandbox_mode or merely advises. Emitted
  TOML is therefore byte-identical for all 35 roles - asserted per role, not in
  aggregate, because an aggregate passes while one role silently widens.

  The hold list self-invalidates. A hold whose role no longer derives broader
  fails, and so does a hold naming a role with no agents/<name>.md. Without
  that it would rot into exactly the hand-maintained subset map being deleted,
  and this commit's own ledger claim would become false over time. Both cases
  were proved by injecting them and watching them throw.

  Two committed tests asserted the deleted map's existence and contents. They
  were pinning the thing being removed, so the tests moved rather than the
  production code: the 11 role-value pairs survive as a test-local
  PRE_3897_CODEX_AGENT_SANDBOX baseline, and the assertions now drive the real
  derivation against real agents/*.md. The coverage is preserved; only its
  source moved out of production code.

  validate agents gains checkCodexSandboxPosture, mirroring the existing
  checkCodexModelPosture: each installed TOML's sandbox_mode must equal the
  role's expected value, failing with role, expected and found. It previously
  checked file presence and manifest completeness only, so a TOML whose
  sandbox_mode disagreed passed.

Rung 4 — shortFormToId, recovered rather than invented.

  I nearly reported this as another wrong §8.3 claim: git log -S returns only
  documentation commits. Wrong instrument. sdk/src/query/phase.ts at 11918dcc3^
  carries five occurrences, and the implementation here matches that code rather
  than a guess at its semantics - including first-write-wins on a duplicate
  short form, deterministic from the sorted plan order.

  It resolves the bare plan number: depends_on: ["01"] now reaches
  26-01-auth-hardening. That is a control-flow change, not a diagnostic one -
  plans that silently collapsed into a single wave 1 now execute in their
  declared waves, and execute-phase.md consumes those wave values.

  In-phase only, by construction: the map is built from this phase's rawPlans,
  so a same-named short form in another phase does not resolve.

  #3785's display-mapping passthrough and #3885's unresolvable-token warning and
  wave-verdict suppression are untouched and stay green. If the third tier had
  over-reached, those are what would have caught it.

Two blockers from review. Both are mine, and one is a security regression my own
change created.

1. A held role could escape its hold by editing its own frontmatter.

  The Codex install loop set the sandbox identity from the agent's frontmatter
  `name:` field rather than from its filename, so the hold lookup keyed off a
  value the file itself declares:

    deriveCodexSandboxMode('gsd-doc-writer',   <real file>)          -> read-only
    deriveCodexSandboxMode('gsd-doc-writer-x', <same file, name: edited>) -> workspace-write
    deriveCodexSandboxMode('GSD-Doc-Writer',   <same file, name: recased>) -> workspace-write

  What makes this a blocker rather than a nit is the DIRECTION. The deleted
  CODEX_AGENT_SANDBOX map had the identical lookup-key quirk, but it was an
  allowlist: an unmatched key fell back to read-only, which is safe. The new
  scheme derives workspace-write from the tool contract and uses the hold as a
  subtraction, so the same mismatch fails OPEN. I converted a fail-closed quirk
  into a fail-open one and did not notice; the isolated reviewer proved it by
  execution.

  Neither safety net caught it. validateCodexSandboxHolds only checks that
  <key>.md exists, never that a file's derived identity matches its key.
  checkCodexSandboxPosture looks the canonical source up by the installed TOML's
  filename, finds nothing for a renamed agent, and treats it as a custom
  non-roster agent — silently no violation.

  The identity is now the FILENAME STEM, which is what validateCodexSandboxHolds
  already validates and what an attacker editing frontmatter cannot change
  without renaming the file — at which point the existing validator catches it.
  The lookup is case-insensitive so a recase does not slip past either. The
  frontmatter name still drives the TOML body and filename, unchanged; only the
  sandbox identity moved.

  All 35 roster files were checked: name matches filename stem everywhere, so a
  stricter "they must agree or throw" invariant would have been safe against real
  content. It is deliberately NOT added — it would abort an install on a tampered
  file where emitting a correctly-derived read-only TOML is the safer outcome.
  Recorded as a fork rather than decided silently.

2. checkCodexSandboxPosture was exported and never called.

  cmdValidateAgents (src/verify.cts) called checkAgentsInstalled and
  checkCodexModelPosture only; grep for the sandbox check in that file returned
  nothing. So criterion 3 — "validate agents fails on semantic drift, not only on
  missing files" — was unmet, and `validate agents` behaved exactly as before.
  That is ADR-3473 Decision 2's named shape: a declared policy with no executor.

  It also meant the T28 test asserted at the helper's return value while the
  COMMAND stayed broken — the ADR-3180 Decision 4(b) failure this epic exists to
  close, committed by me while enforcing it elsewhere in the same epic.

  Now wired as an additive `sandbox_posture` field beside `codex_posture`,
  following the sibling precedent exactly. Drift is report-only, not a non-zero
  exit, because that is what checkCodexModelPosture does — two sibling posture
  checks disagreeing about whether a violation is fatal would be its own defect.
  The choice is recorded in a comment rather than left implicit. A consumer-output
  test now drives the real CLI and asserts on the emitted JSON, and was shown
  failing before the wiring and passing after.

Also corrected a stale artifact: the design's Known limit L1 still claimed rung 3
was not in this deliverable, written while it was halted and false once the
maintainer unblocked it.

Verified after both fixes: the three bypass probes all return read-only, the
per-role table is 35/35 byte-identical, and both hold self-invalidation cases
still throw.

presence only, and the semantic assertion lives in a sibling wired into validate
agents rather than folded into it.

No how-to. Both user-visible changes are zero-step: a non-Claude install resolving
its own runtime, and plans executing in their declared waves, both happen without
the user doing anything. docs/how-to/control-the-reported-host-runtime.md covers a
DIFFERENT ladder (resolveReportedRuntime / agent_runtime) that this change does
not touch, and was deliberately left alone rather than edited by association.

No tutorial - nothing multi-step to walk through. docs/AGENTS.md unchanged: it
documents Claude-side tools frontmatter, never Codex sandbox_mode, and the
emitted tools contract did not change.

The prompt layer documents depends_on only by example, not by schema, so nothing
there needed editing - and few-shot-examples/plan-checker.md already showed
depends_on: ['01'], which now actually resolves.

Translated copies of plan-md.md are untouched; the project treats translations as
community-maintained.

The full suite came back with 26 failures across four files. Three distinct
causes, mapped individually rather than assuming the first explained the rest.

A. Requiring bin/install.js printed the GSD banner to stdout and corrupted
   `validate agents` JSON.

     Unexpected token '', "[36m   ██"... is not valid JSON

   checkCodexSandboxPosture reached deriveCodexSandboxMode by lazily requiring
   bin/install.js, whose module load prints the ASCII banner. So the command
   emitted banner bytes before its JSON and every JSON consumer broke, including
   ten tests that predate this branch. src/ reaching into bin/ was backwards
   layering that happened to also be loud.

   The derivation now lives in src/codex-agent-toml.cts - the existing Codex TOML
   domain module, no new module and no six-gate ripple - and both bin/install.js
   and src/agent-install-check.cts import it. One owner, which is §8.3's rule
   applied to the fix for §8.3.

B. The stale-hold throw fired on a legitimate partial source dir, and masked a
   security assertion.

   validateCodexSandboxHolds treated "this hold's .md is absent from the install
   SOURCE dir" as a stale hold and threw. A test fixture, or any partial install
   source, legitimately contains a couple of agents. Worse, it threw BEFORE the
   path-escape check, so a test asserting that a `../../evil` frontmatter name is
   rejected got my unrelated error instead of the traversal rejection it was
   written for. A fail-closed check of mine was hiding a real security check.

   The "no stale holds, shrink-only" invariant is a property of the repo's
   canonical agents/ roster, not of whatever directory an install happens to read.
   It is off the runtime path and enforced where it belongs, in the tests that
   already existed for it. A partial source dir now installs cleanly, and the
   evil-name case throws with its own escapes-configHome message again.

C. T8 depended on ambient process.env state.

   The marker/env parity assertion round-tripped through live process.env. It now
   compares against resolveExplicitRuntime's already-exported dependency-injection
   parameter - deterministic and hermetic, same claim. Proven still falsifiable
   rather than assumed: with the marker rung's normalization temporarily bypassed
   the two rungs diverge ("codex\n../../etc/passwd" vs "codex-../../etc/passwd")
   and the assertion fails, then passes again once reverted.

One correction folded in along the way. The first version of the move added
private _extractFrontmatterAndBody/_extractFrontmatterField helpers to
codex-agent-toml.cts - a THIRD copy of frontmatter extraction, where the graph
already shows two (bin/install.js:2348, runtime-artifact-conversion.cts:893).
Adding a third inside the epic whose thesis is one implementation per rule is not
defensible. deriveCodexSandboxMode no longer parses anything: it takes
(identity, toolsValue) and each caller supplies the tools value using the
extractor it already has. Both helpers are deleted. The identity argument is
still the filename stem, so the fail-open fix is untouched.

Verified after all three: `validate agents --raw` emits parseable JSON with no
banner and both posture fields; the four hold-bypass probes still return
read-only; the per-role table is 35/35 byte-identical at 26 read-only / 9
workspace-write; the hold list is still 16.

A. My extractor import dragged in a script that does not exist in an installed
   tree.

     Cannot find module '../../../scripts/fix-slash-commands.cjs'

   Chain: src/agent-install-check.cts imported runtime-artifact-conversion.cjs,
   which requires command-roster.cjs, whose line 36 requires
   ../../../scripts/fix-slash-commands.cjs. That path exists in the repo and not
   in an install, so every test exercising a synthetic install dir died at module
   load. I picked that extractor for convenience without checking what it pulls
   in - the same mistake that produced the banner bug, one layer further out.

   agent-install-check now uses a single-purpose extractToolsLine on
   codex-agent-toml.cts. That is deliberately NOT a general frontmatter parser:
   we deleted those helpers a commit ago for good reason, and this reads one
   line. Verified from outside the repo root that requiring either module prints
   nothing and does not throw.

B. A test pinned the deleted name-based fallback.

   'defaults unknown agents to read-only' called generateCodexAgentToml with a
   fixture declaring tools: Read, Write, Edit. Under derivation an unknown agent
   with a writing contract correctly derives workspace-write - design row S6, a
   new writing role gets the contract, not the pin. The behavior it asserted was
   the silent fallback this rung deleted; identity no longer decides the sandbox.

   Replaced with two rows rather than a flipped string: no tools declared ->
   read-only (absence is not a grant), and Write/Edit declared -> workspace-write.
   Strictly more coverage than the row it replaces.

C. The stale-hold check still threw per derivation call.

   Last commit took the roster-existence check off the install path, but
   deriveCodexSandboxMode itself still threw when a hold's role did not derive
   broader FOR THE CONTENT IT WAS HANDED - so it fired on any synthetic fixture
   for a held role.

   The throw is gone, and it cost nothing: if a held role's content does not
   derive broader, the hold pins read-only and derivation returns read-only
   anyway, so the hold is a no-op and there is nothing to fail about. The
   staleness invariant is a property of the real agents/ roster, and
   validateCodexSandboxHolds still enforces it there - confirmed against the real
   roster after the change, not assumed.

   deriveCodexSandboxMode is now total: every (identity, toolsValue) including
   undefined and null returns read-only or workspace-write, never throws.

Verified: validate agents emits parseable JSON; the four hold-bypass probes
return read-only; the per-role table is 35/35 at 26 read-only / 9
workspace-write.

directory is gitignored (.gitignore:55), so the rationale for holding 16 roles at
read-only was reachable only from the machine that produced it. A reader of the
ADR got a pointer to nothing.

Both now carry the reasoning inline: the criterion asks both that the sandbox
derive from the declared tool contract and that no role gain a broader sandbox,
and those cannot both hold, because a faithful derivation widens 16 roles the
deleted map never listed and that fell through its silent read-only default. The
resolution is derive-and-hold - the derivation owns the rule now, each hold is
released as its enforcement question is answered, and a hold is reversible where
a widened sandbox that turns out to be enforced is not.

Checked before assuming this was a defect class: CONTEXT.md cites
.gsd/phase/<slug>/40-design.md as its standard Design: provenance line in eight
module entries, and four other shipped docs do the same. Citing a phase artifact
is an established convention here, so those are left alone. What was wrong was
specific to these two: they put load-bearing rationale behind the pointer instead
of provenance.

docs/FEATURES.md regenerated from the fragment via scripts/gen-features.cjs
rather than hand-edited.

Two orthogonal reviews on the shipped sha. Three of the findings are the same
failure class this epic exists to close, committed inside it.

1. BLOCKER - the sandbox was decided for one identity and applied to another.

   bin/install.js derived sandbox_mode for the filename stem and then wrote the
   result to `${name}.toml`, where name comes from the file's own frontmatter.
   Make the two disagree and a HELD role's artifact goes wide:

     rename gsd-doc-writer.md -> gsd-doc-writer-v2.md, keep name: gsd-doc-writer
       -> stem is unheld, derives workspace-write, lands on gsd-doc-writer.toml
     add any gsd-*.md whose frontmatter name: is a held role
       -> clobbers that role's toml with workspace-write

   Both emit read-only on origin/next, because the deleted map was an allowlist
   and a miss fell back safe. This is a regression my change introduced. The
   previous review round moved the HOLD KEY off frontmatter to the filename stem
   and left the OUTPUT PATH on frontmatter; my own comment at install.js:6985
   calls that value attacker-editable, four lines above the line that uses it as
   the filename.

   The decision is now made over BOTH candidate identities, most-restrictive
   wins: if either the stem or the emitted name is held, the mode is read-only.

2. MAJOR - hold matching was toLowerCase() only, so confusables escaped.

   Turkish dotted/dotless i, fullwidth, NFD, trailing space/NBSP/dot/newline,
   ./ and ../agents/ all slipped the hold and emitted workspace-write.
   Identities are now basenamed, trimmed of NBSP/zero-width/control characters,
   NFKC-normalized and lowercased - and anything still carrying a character
   outside [a-z0-9._-] is treated as suspicious and derives read-only. We do not
   enumerate confusables; every shipped roster file is ASCII, so refusing to
   widen on an identity we cannot recognize is fail-closed with no false
   positives on real content.

3. MAJOR - the short-form depends_on tier mis-resolved SILENTLY.

   shortFormToId keyed on the last dash-segment of any canonical id with no
   constraint that it is a plan number, so a phase holding 09-FIX-auth-PLAN.md
   made depends_on: ["auth"] bind at wave 2 with zero warnings. This is the
   worst shape in the epic: the unresolvable-token warning fires on a DROPPED
   token, so a MIS-RESOLVED one is invisible and the tool reports a confident
   wave assignment built from a wrong edge. A wrong edge is worse than a missing
   one.

   The segment must now match /^\d+$/, which is exactly the contract
   docs/reference/plan-md.md already documents. This tier was recovered verbatim
   from the retired SDK lineage, which carried the same defect; we are
   deliberately NOT preserving it bug-for-bug, and the comment says so, so the
   next reader does not "restore" it.

4. MAJOR - the derivation was reading a declaration as an absence.

   extractToolsLine read one line, so a YAML list-form tools: block returned only
   its first item. Two roster files use list form, and gsd-nyquist-auditor
   declares Write and Edit there - parsed as "- Read", found no write tool, and
   emitted read-only. Rung 3's headline claim is that sandbox_mode derives from
   the declared tool contract; that claim was false for 2 of 35 roles and
   materially wrong for 1. Reading a declaration as an absence is the silent-drop
   class this epic exists to close.

   Renamed extractToolsValue and taught it both shapes. gsd-nyquist-auditor now
   derives workspace-write and joins CODEX_SANDBOX_HOLDS as its 17th entry, per
   the standing derive-and-hold decision - so emitted TOML stays byte-identical
   at 26 read-only / 9 workspace-write while the hold list finally records every
   role that would widen. A previous pass declined this fix because it moved the
   count; that inverts the priority. Byte-identity is preserved THROUGH the hold,
   not by leaving a parser broken.

   Divergence check, because this is where that bug hides: both paths feeding
   sandbox derivation - install.js's emitter and checkCodexSandboxPosture - now
   route through the one extractor. The tools readers in
   runtime-artifact-conversion and install.js's other frontmatter call sites
   serve Claude-side emission and do not feed sandbox derivation.

Also fixed, each real: the posture check's `found` used a naive whole-file regex
where its own sibling uses the block-aware scanner, so prose inside
developer_instructions produced a false violation; `found` skipped
truncatePostureValue and leaked a 300-char value into validate agents output;
deriveCodexSandboxMode's absolute never-throws claim was false for an object with
a throwing toString; T49 could not falsify cross-phase leakage (its target phase
had its own 01, so a globally-scoped map passed too); T20/N6 iterated a hardcoded
table and pinned the FIXTURE size, so a 36th agent would be silently unchecked;
three tests reimplemented the code they were testing instead of importing it; and
T2-T4 deleted GSD_RUNTIME without restoring it.

Verified: hold list 17, gsd-nyquist-auditor derives workspace-write unheld and
emits read-only held, roster 35/35 at 26/9, depends_on ["auth"] no longer
resolves while ["01"] still does, both identity-bypass cases and every confusable
vector emit read-only.

The installer exported 197 names and had zero production consumers - every
non-test require of it repo-wide sits inside a comment. Its interface was
shaped by test access, not by callers.

Removes 9 dead exports and 61 pass-throughs, repointing their tests onto the
extracted modules' own interfaces. 197 down to 127.

Every count in the issue was wrong: 197 exports not 188, 9 dead not 12, 61
pass-throughs not 49, 44 test files not 42 - and the audit itself then missed
7 more consumer files. restoreUserArtifacts was on the dead list but ceased to
exist in phase 6, and two _GSD_EFFORT_MANIFEST_* names listed as dead are now
genuinely asserted, so acting on that list would have deleted live exports.

7 of the 9 dead names collide with an independent declaration that install.js
delegates TO. Each removal was justified by which declaration a reference
resolves to, never by whether the name appears somewhere.

Coverage parity was the gate rather than test greenness: per-file counts were
captured before any edit and diffed after. 44 of 45 files are byte-identical;
the single delta is one added assertion, not a loss.

The sweep for scattered require sites found two forms static grep misses -
require(VARIABLE) and multi-line require() - plus tests asserting that
install.js re-exports the SAME object, which now assert retirement instead.

Both review engines found real defects in the first cut.

The DEFECT.GENERATIVE-FIX single-owner guard from #1511 had been repointed
from a reference-identity check to install.X === undefined. Those are not
equivalent: the guard exists to catch a duplicate function body reintroduced
into install.js, and the replacement passes cleanly if that duplicate is used
internally and never exported. It now walks bin/install.js's real top-level
bindings, so it catches a duplicate under either shape, exported or not -
strictly stronger than the check it replaced. Proved by injecting a duplicate
and watching it go red.

That weakening survived the coverage-parity gate because the assertion count
never moved. The gate compares counts, so an assertion that changes meaning
rather than number is invisible to it.

Removing the exports had orphaned their wrapper bodies: 14 dead wrappers, 9
consts and 9 destructure entries, several pre-existing and found by the same
sweep. Dead code left in the file this phase exists to shrink.

Three more comments claimed re-exports this phase removed, and tests were
reading Cursor and Windsurf hook constants from install.js's local copy while
calling functions from the hooks surface - equal today, with nothing holding
them equal. The local consts now reference the owning module.

dc3c81e93d chore(#3212): src/pattern.cts is the sole owner of runtime-value regex construction — Phase 1 (#3416)
* test(#3412): failing-first suite for the pattern-construction seam

Phase 1 of epic #3212 (ADR-3212 §1/§2/§7). Tests only — src/pattern.cts
and eslint-rules/no-adhoc-regex-escape.cjs do not exist yet, so both
suites fail with MODULE_NOT_FOUND, which is the intended RED.

Locks the measured behavior rather than the assumed behavior:
RegExp.escape hex-escapes the leading character of nearly every string
("abc" -> "\x61bc"), so the suite asserts match-equivalence against an
inlined historical oracle (the implementation being deleted) rather
than byte-equivalence of pattern text — 200 seeded fast-check runs plus
a fixed corpus, 0 mismatches. Also locks the latent character-class
range bug this phase fixes as a side effect: a hyphen-bearing value
interpolated into [...] currently forms a real range and matches an
unintended character; post-migration it must not.

* chore(#3412): src/pattern.cts owns runtime-value regex construction

Phase 1 of epic #3212 (ADR-3212 §1/§2/§6/§7). Adds the pattern seam
delegating to the built-in RegExp.escape, deletes every hand-rolled
copy, and raises the Node floor to the Active LTS line.

The census was low, three times over. ADR-3212 counted 10 copies; a
graph query found 12; the new lint rule — once live — found 27 more.
The difference is that the census counted named helper FUNCTIONS while
the rule counts the escape SHAPE, so inline .replace(<class>, '\$&')
copies were never in scope. ADR §1's actual requirement is that no
module outside the seam escapes a value for regex use, so all of them
are, and CLAUDE.md's no-defer rule makes them this change's work.
Fourth consecutive epic here whose copy count was low — the argument
for ADR-3180 Amendment 3's "state N found by the guard" rule.

Also corrected mid-implementation: the survey reported phase-id.cts's
escapeRegex had 0 external importers. It had 8 production importers,
making its removal a public-surface change to an ADR-2121-owned module
and requiring an update to that ADR's locked-surface test. Blast
radius revised Medium-High -> High.

RegExp.escape is match-equivalent but NOT text-equivalent: it
hex-escapes the leading char of nearly every string ("abc" ->
"\x61bc"). Equivalence is proven by a seeded fast-check property test
against the deleted implementation as oracle. It also fixes a latent
bug: a hyphen-bearing value interpolated into a character class
previously formed a real range and matched an unintended character.

Node floor 22 -> 24 (RegExp.escape is Node 24+), across engines,
.nvmrc, package-lock, 9 CI matrix entries, and 5 docs. The aggregate
`required-tests` context is unchanged and no job was added or removed,
so branch protection cannot be orphaned by the dropped lanes.

Enforced by eslint-rules/no-adhoc-regex-escape.cjs (shape-matched, with
structural provenance for reviewed pattern-fragment constants rather
than a name heuristic) plus a whole-tree companion guard covering the
directories ESLint's globs miss.

* fix(#3412): close the _SOURCE guard evasion, correct two false claims

Three findings from the orthogonal review pass, all fixed.

1. The ESLint rule's `_SOURCE` provenance fallback was pure identifier-
   name matching with no binding check, so `new RegExp(userInput_SOURCE)`
   — a function parameter — sailed past the guard. That is the same
   rename-evasion class issue #3410 documents, reopened by the very
   fallback meant to complement the structural check. Now bound to the
   identifier's actual binding kind: import, require-derived const, or
   module-scope const; parameters, `let`/`var`, and unresolvable
   bindings fail closed. Four RuleTester cases cover the evasion and
   prove the legitimate cross-module case still passes.

2. src/pattern.cts's own header carried the stale pre-correction counts
   (12 copies / 17 call sites) while CONTEXT.md and the design doc
   carried the corrected ones (~39 / ~44) — a self-contradiction inside
   the PR whose entire purpose is deleting divergent copies. Rewritten,
   preserving the durable lesson: a named-function census cannot see
   inline copies; only a shape-matching guard can.

3. The claim that all deleted copies threw TypeError on non-string was
   false. phase-id.cts's copy — the one with 8 external importers — did
   String(value).replace(...) and never threw. The seam's locked
   signature does not coerce, so this is a real, now-disclosed behavior
   change rather than the pure preservation the tests asserted. Audited
   all 32 invocations across the 8 importers and 6 in-file callers:
   every one is safe by construction (upstream truthy guard or a
   string-producing derivation), verified by runtime probe against the
   compiled modules rather than by TS compilation, which cannot see a
   runtime undefined. Corrected the false claim in both the test comment
   and the design doc, and added it to Known limits.

* docs(#3412): add Changed changeset for the Node 24 floor

The only user-visible break in this phase. The escape-behavior change
is internal and match-equivalent, so it carries no user-facing note.

* fix(#3412): resolve the seam's require graph in script fixtures and packaging

Checkpoint 2 came back red with 90 failures on the node24 lane. Three
distinct defects, all introduced by routing scripts/ through the new
pattern seam, none reproducible by any local gate:

1. ~82 failures — tests/adr-index-gate.test.cjs and
   tests/removed-but-needed-lint.test.cjs copy a scripts/*.cjs into an
   mkdtemp fixture and spawn it there (necessary: those scripts resolve
   their scan root from __dirname/.., so running the real script would
   scan the real repo). Each harness hand-listed the dependencies to
   copy alongside. Adding require('../gsd-core/bin/lib/pattern.cjs') to
   gen-adr-index.cjs made both lists silently incomplete ->
   MODULE_NOT_FOUND, plus 17 downstream 'did not emit parseable JSON'
   failures from the same crash.

   Fixed as a class, not an instance: new tests/helpers/copy-script-
   fixture.cjs walks a script's transitive static relative-require graph
   and copies it, so dependencies are derived and never re-declared. It
   throws (naming the unbuilt artifact) instead of letting the child die
   with a bare MODULE_NOT_FOUND. Verified for all four seam-consuming
   scripts: gen-adr-index, lint-removed-but-needed, gen-loop-host-
   contract, sync-runtime-launcher.

2. 2 failures — scripts/ ships wholesale but eslint-rules/ does not, so
   the new scripts/lint-no-adhoc-regex-escape.cjs would be
   MODULE_NOT_FOUND in a published install (#2858 guard). Excluded from
   the tarball, matching the existing precedent for gen-emitted-
   baseline.cjs, which is excluded for the identical reason, and locked
   with a test modeled on that one. Confirmed against a real npm pack:
   890 files, 0 from eslint-rules/, and gsd-core/bin/lib/pattern.cjs
   present (so the other four scripts' requires are legitimate).

3. 6 failures — tests/phase-id.test.cjs asserted the literal escaped
   source text ('0*29', 'PROJ-42'). RegExp.escape is match-equivalent to
   the retired hand-rolled escaper but NOT text-equivalent: it hex-
   escapes the leading character and all hyphens ('0*\x329',
   '\x50ROJ\x2d42'). Verified NOT a behavior change — 576 match
   decisions across all three real interpolation prefixes, zero
   divergence. Those tests now compile each source into the same heading
   regex src/roadmap.cts's searchPhaseInContent builds and assert what
   matches and what does not, including the 'i'-flag canonicalization
   the hex escape has to preserve. Re-pinning the new literals would
   have rebuilt the same brittleness one layer down. Adds a test for the
   property the escape exists for: a dot in '1.2' must not act as a
   wildcard.

Also shares one definition of 'a require' between the packaging guard
and the fixture copier, so the two cannot disagree about what they scan.

copyScriptWithDeps resolved each relative require and joined the
repo-relative result onto fixtureRoot. A require resolving OUTSIDE the
repo yields a '../'-prefixed relative path, so path.join climbed out of
the fixture and wrote into the surrounding temp dir (verified:
repoRoot=/repo + depAbs=/etc/passwd wrote /tmp/etc/passwd).

No script in the tree does this today, so this closes an available
escape rather than an active one. Refuses via the existing unresolved-
require path so the failure names the offending specifier. Covered by a
negative proof that the guard fires and that nothing lands outside the
fixture.

Applies all findings from the second orthogonal review round, re-run
because real code changed after round 1.

HIGH (security) — extractRequires stripped BLOCK comments before LINE
comments, so a '//' comment containing '/*' opened a phantom block
comment, and a '//' inside a string literal truncated the line. Both
hid real requires: 'const u="http://x"; require("./real.cjs")'
returned [], and four real requires in gsd-core/bin/gsd-tools.cjs were
invisible. Replaced with a real AST parse via espree.

This is ADR-3212's own Decision 4 — tokenizer-first for stateful
grammars — applied to the case it describes; comment/string/regex
nesting is exactly such a grammar, which is why the regex version was
wrong. The function was moved byte-identical out of the #2858 packaging
guard, so the bug PRE-DATES this branch and has been a live blind spot
there: a shipped script could have required an unshipped path
undetected. Fixing it makes that guard strictly stronger than on next.

espree is promoted from a transitive eslint dependency to an explicit
devDependency rather than relying on hoisting. The script parse attempt
sets ecmaFeatures.globalReturn because Node wraps CommonJS bodies in a
function, making a top-level return legal — scripts/check-coverage-gate
.cjs relies on it, and without the flag the guard throws on a file it
is supposed to scan. Verified 0 unparseable across all 324 .cjs/.js
under scripts/, bin/, and gsd-core/bin/, and 0 new violations against a
real npm pack, so the exact extractor does not newly fail the guard.

MEDIUM (security) — the repo-containment check guarded dependencies but
not the entry path. One escapesContainment predicate now guards both.

LOW (security) — containment was lexical while fs follows symlinks, and
a directory symlink could mint a fresh dedupe key per level. realpath
now resolves both repoRoot and each dependency before the decision, and
the realpath-derived path is the dedupe key. Destination layout still
uses the original repo-relative path, so copied trees are unchanged.

MAJOR (standards) — the round-1 behavioral rewrite of phase-id tests
lost the foreign-prefix contract: every assertion was satisfied by an
impl returning [A-Z]+\x2d42, i.e. ANY project code — the exact #3599
bug class the exact-source prevents. The literal assertions it replaced
were catching this. Now asserts the compiled regex REJECTS a different
prefix with the same number.

MAJOR (standards) — the test hand-duplicated production's heading regex
with no parity guard (CLAUDE.md's 'Generative Fix Divergence'). Removed
the parallel surface instead of policing it: src/roadmap.cts exports
buildPhaseHeadingRegex, searchPhaseInContent calls it, the test imports
it. Byte-identical .source and .flags verified for both escaped forms.

MINOR — '..foo' no longer false-flagged as an escape; the inverted
spurious-vs-missing doc claim corrected; the dead allow-test-rule
header removed.

Two CI failures on PR #3416, both in code this branch added.

CodeQL js/redos (high) — REPLACE_CALL_RE's outer alternation let a
bracket run be consumed EITHER by the character-class branch OR one
character at a time by the trailing catch-all, so a failing match
explored both parses of every pair. Measured on the real regex:
n=26 -> 204ms, n=28 -> 791ms, n=30 -> 3475ms, a clean 2^n. This script
scans repo source, so a file with a long bracket run after '.replace(/'
would hang CI outright — a guard against undisciplined pattern
construction was itself the worst pattern in the diff.

Fixed the way ADR-3212 already prescribes: the catch-all branch now
excludes '[' and ']' so a bracket can only be consumed by the class
branch (this is what makes it linear), and every quantifier is bounded
(the locked bounded-quantifiers decision) as a second line of defense.
Now 0ms at n=2000. Disclosed coverage tradeoff, recorded at the
constant: a regex literal with a BARE unescaped ']' outside a class is
no longer matched by this backstop. No census shape has that form, and
the AST rule remains the primary detector.

Verified the guard did not go blind doing it: a real census-shape
violation is still reported, and an allow-adhoc-regex-escape
suppression comment is still honored.

Regression test drives the exported findViolations on a
2000-repetition adversarial input and asserts the RESULT. It makes no
wall-clock assertion — elapsed-time tests are forbidden — so a
regression surfaces as a harness timeout, which is the correct signal.

Prompt injection scan — 'must not act as a regex wildcard' in a test
comment matched the scanner's jailbreak pattern act\s+as\s+(a|an|if|
my). Reworded to 'behave as'. Deliberately NOT allowlisted: silencing a
whole test file over one phrase would blunt the scanner permanently,
and the comment has nothing to do with injection.

Neither failure was reachable from the remote runner — CodeQL and the
injection scan are not in that matrix, so the sha it passed was green
and still wrong.

their module's main suite, per H3 (#3315) of the test-hygiene epic (#3053).
First of 4 issue-* waves (following the 3 fix-* waves, all merged).

- 1 file with no prior target coverage: renamed (git mv) into
  legacy-cleanup.test.cjs (sole comprehensive suite for that module).
- 9 files merged into 6 pre-existing suites: golden-parity-single-source,
  runtime-artifact-layout-surface, codex-config (4 sources merged jointly
  in one pass per the issue's own instruction, to catch overlap between the
  4 sources themselves, not just against the pre-existing target — zero
  overlap found, all 20 blocks additive), runtime-config-adapter-registry
  (1 of 10 source blocks dropped as a proven subset of existing coverage),
  cline-install, install.test.cjs.

Incidental fixes required to keep this wave's own ratchets green:
- Fixed a stale ADR doc reference (docs/adr/1235) to a folded-away filename.
- scripts/lint-allow-test-rule-refs: pruned 4 stale allowlist entries for
  renamed/merged-away files, cited 2 previously-uncited allow-test-rule
  comments that surfaced as "new" only because their file path changed,
  added 1 fresh allowlist entry for a pre-existing uncited comment that
  predates this PR, and tightened the exemption-file ceiling 309 -> 305
  to match the real post-fold high-water mark.

Zero net test-coverage loss. No production code changed.

* test(#3336): fix orthogonal-review findings — Wave 4 fold

Standards-axis review + Memtrace graph pass found real issues in the
just-folded suites, all fixed here:

- Standardized the fold-wrapper convention (block-scoped __foldDescribe)
  across golden-parity-single-source.test.cjs, runtime-artifact-layout-
  surface.test.cjs, runtime-config-adapter-registry.test.cjs, and
  cline-install.test.cjs to match the pattern already used by
  codex-config.test.cjs and install.test.cjs in this same wave (and by
  earlier folds elsewhere in the epic) — repeats the exact inconsistency
  Wave 3 (#3335) already fixed once in this epic.
- Fixed a stale allowlist entry's alphabetical position (cosmetic, not
  tool-gated, caught by review anyway).
- Fixed two stale test-filename references in PRODUCTION code comments
  (src/capability-writer.cts, src/runtime-config-adapter-registry.cts)
  caught by lint-removed-but-needed — a class of stale reference this
  wave's fold agents didn't check for, since they were scoped to docs/
  and gsd-core/references/ only, not src/. First fix attempt wrongly
  edited the gitignored gsd-core/bin/lib/*.cjs BUILD OUTPUT instead of
  the tracked .cts source; caught and corrected before commit.
- Fixed one remaining stale doc reference in docs/adr/1235 (a prior
  partial fix in this same wave missed it).

No test() count changed in any file. No production code BEHAVIOR
changed — comment-only fixes in src/.

58d73dd220 enhance(#3241): omit the codex per-agent model by default (#3276)
* test(#3241): failing-first suite for the codex passive model posture

Locks ADR-2313's D1-D5 before any production code exists, so the tests
bind to the behavior rather than to whatever the implementation happens
to do.

Red-first (fail against the current tree):
  - the resolver path emits no `model` and no `model_reasoning_effort`
  - a whitespace-only model_overrides value yields no pin
  - isAnthropicFlavoredModel / CLAUDE_AGENT_ALIASES on model-catalog
  - the one-time install notice, and its once-per-install dedupe

Regression guards (pass today, must keep passing): resolver-null via
`inherit` and via absent runtime; a resolver that resolves to nothing;
empty-string and non-string overrides; and the light-tier
service_tier/model_verbosity fields, which are NOT coupled to the model
pin and would silently regress if the implementation coupled them.

Classifying each test as red-first or regression guard is deliberate.
A test that passes on both sides of the change proves nothing, and this
epic has already shipped two such rows before catching them.

The whitespace case is a live defect, not a quirk: `'   '` is truthy,
survives the type guard, is not Anthropic-flavored, and is embedded
verbatim as `model = "   "` — the same class the #2310 guard exists to
stop. Same function, same path, fixed in this phase per CLAUDE.md §3.

Two matrix rows were dropped as vacuous rather than shipped green: a
64-char truncation case (the pinned notice interpolates no
user-controlled value, so it cannot exhibit truncation) and a newline
hazard that the input surface cannot reach.

Implements ADR-2313 D1-D5. generateCodexAgentToml no longer embeds the
runtime resolver's per-tier Codex model, so an agent inherits the
always-available session model instead of a pin a ChatGPT-account Codex
may not expose. model_reasoning_effort disappears with it via the
existing hasPinnedModel coupling (#838) — no logic change needed there.

Supersedes #2517's embedding on the default path only. An explicit
real-Codex model_overrides pin is still embedded verbatim, and the #2310
Anthropic-flavored guard is retained: the model_overrides route to it is
still live even though the resolver route is now unreachable.

The shared rule moves down a layer. CLAUDE_AGENT_ALIASES leaves
model-resolver for model-catalog — a genuine leaf importing only
node:path and its own JSON — with isAnthropicFlavoredModel defined beside
it, and is re-exported from model-resolver so every existing importer is
untouched. This is what lets Phase 2's install-check and Phase 3's sync
consume the rule without taking the config-loader dependency
model-resolver would have dragged into a module documented as pure
read/verify with 33 dependents. A parity test fails if the two ever fork.

Also fixes a live defect surfaced while writing the tests: a
whitespace-only model_overrides value was truthy, survived the type
guard, was not Anthropic-flavored, and so was embedded verbatim as
`model = "   "` — the same class the #2310 guard exists to stop, reached
by a different route. Trimmed before the truthiness test. It is
deliberately not routed through _warnCodexModelOverrideDropped, whose
text would misdescribe a blank field as a mis-typed model.

Adds the one-time install notice (maintainer direction, recorded as an
ADR-2313 amendment): one stderr line naming model_overrides and the
session model, deduped per install rather than per agent, and emitted
only for the population that actually loses a pin.

service_tier and model_verbosity stay decoupled from the model (#774);
a regression guard asserts they still emit with nothing pinned.

ADR-2313 gains two dated amendments rather than edits to its merged
text, since ADRs here are append-only.

The first records that a deprecation notice IS offered, reversing the
Migration section's "no deprecation window" position, and states why
that position was wrong rather than just superseding it: the ADR
identified the API-key population as losing something real and then
declined to warn it, in the same document. Hyrum's guidance was applied
to the recourse and not to the notice.

The second records the whitespace-only model_overrides defect and notes
that D2 always implied the fix — the implementation simply never
enforced it and no test covered the case.

CONTEXT.md gains a Model Catalog Module entry. The module had none,
which is why the glossary gate passed without one: check-glossary-refs
verifies that references resolve, not that modules are documented. The
entry records why the Anthropic-flavored rule lives there rather than in
model-resolver, so a later reader does not "helpfully" move it back. The
Model Resolver entry is updated to point at its new home and note the
back-compat re-export.

docs/CONFIGURATION.md carried a claim that is now false: that the
resolved tier ID is embedded into agent frontmatter at install time on
codex and opencode. Corrected to name codex as the exception, with the
400 symptom and the model_overrides recourse.

Changeset leads with the user-visible change and the migration line
rather than the implementation, per the ADR's Hyrum's-Law analysis.

Review finding from an isolated reviewer. The deprecation notice gated
on whether the runtime resolver would have returned *any* model, but the
question that matters is whether that model would have been *embedded*.

Those differ. The #2310 safety gate already rejected an Anthropic-
flavored model arriving from the resolver path before Phase 1 — so for a
mixed-runtime config resolving to a claude-* id against a Codex install
target, the user never had that pin. The notice told them they lost
something they never got, and pointed them at model_overrides for no
reason.

The existing #2310 test drives exactly that path but asserts only the
emitted `model` line, never stderr, which is why it slipped through. Now
covered.

Deliberately unchanged: an Anthropic-flavored model_overrides value plus
a legal resolver model fires BOTH the override warning and the notice.
That is correct — pre-Phase-1 the guard dropped the override, execution
fell through to the resolver, and the resolver's model was embedded, so
that user did lose a pin. Two messages, two distinct true facts, and the
prefixes differ (`gsd: warning — ` vs `gsd: notice — `) so the
one-notice-per-install contract holds. A regression test now pins that
behavior so it does not get "simplified" away.

Of the three tests added, only the first is red-first; the other two
pass on both sides by design and are labelled as guards — one against
over-correcting the fix into silence, one against removing the
intentional double message.

The remote runner caught a regression I introduced: the #2760
post-write-validation test began failing with the validator override no
longer intercepting.

Cause, confirmed by trace rather than guessed: the new #3241 review
tests deleted require.cache for bin/install.js and re-required it mid
suite, to clear the notice's module-level dedupe flag. But
runCodexInstall destructures `install` at file load, closing over the
ORIGINAL module's exports. After the cache bust a second instance
existed, so the test's `installModule.__codexSchemaValidator = ...`
mutated the new object while the code under test still called the old
one. The override silently stopped intercepting, the real validator ran
and passed on GSD-emitted output, and the abort-and-restore path was
never exercised.

Cache-busting a module mid-suite breaks every later test that assumes a
single instance, which every other test in the file is entitled to. So
the fix is a seam, not a workaround: bin/install.js exports
_resetCodexNoticeDedupeForTests(), and the three tests call it directly
instead of reloading the module.

The flag is module-level by design — the dedupe is per-install and
install() already resets it — so a unit test driving
generateCodexAgentToml directly needs an explicit way to reset it. That
is now what it has.

Swept the rest of the #3241 diff for the same hazard; this flag was the
only shared module-level state introduced.

TWO module-level dedupe stores and the require.cache bust I removed had
been papering over both; my replacement seam cleared only one.

_codexModelOverrideDroppedWarned is a Set keyed `${agent}::${value}`.
tests/codex-config.test.cjs:558 already emits for `gsd-executor::sonnet`,
so by the time the review test using the same agent and value ran,
_warnCodexModelOverrideDropped was a silent no-op and the expected
warning never appeared.

The seam now clears both stores and is renamed to say so. Its comment
records that per-install dedupe lives in module scope deliberately and
that this is the single sanctioned way for a unit test to clear it.

Swept bin/install.js for every other module-scope mutable a test could
latch. Two are inert (capability registries assigned once at require
time; selectedRuntimes computed once from argv). One is a genuine latent
hazard and is deliberately NOT folded in: attributionCache (:1654)
memoizes getCommitAttribution by runtime name for process lifetime, so
two in-process installs of one runtime with differing attribution config
would collide. It is unreachable from any current test and is a
different concern from Codex warning dedupe, so it stays out of this PR
rather than widening it.

The docs gate forced the task-oriented quadrant and found the worst
defect in this change's documentation surface.

docs/how-to/configure-model-profiles.md carried a section titled "If you
want tiered models on Codex" telling users to set runtime:codex +
model_profile:balanced, promising "GSD resolves each tier alias to the
Codex-native model and reasoning effort defined in the runtime tier
map." That is exactly the behavior this PR removes — a how-to page
confidently instructing users to do something that no longer works,
which is worse than a missing page because it fails at the moment of
use.

Rewritten to state that Codex does no tier routing, give the
model_overrides pin as the supported alternative, and name the two
constraints on what may be pinned: it must be a real Codex model id, and
the account must actually expose it — GSD cannot verify the second, so
the honest advice when unsure is to omit the pin. Carries an upgrade
note for both account types, since the change is a no-op for ChatGPT
accounts and a real loss for API-key ones.

Also tightened the same page's claim that Codex "embeds the resolved
model" at install time — now true only of an explicit override. The
re-install instruction it supports is still correct and still needed, so
only the premise moved.

Both the required-docs set (COMMANDS.md + FEATURES.md) and
lint-docs-required.cjs would have passed before this commit, since
CONFIGURATION.md and the ADR had already moved. Neither checks the
quadrant a user in trouble actually opens.

Migrates 156 unbounded sync spawn sites across 47 files. Allowlist 120 to 73.

Timeouts are sized from evidence already in the tree rather than a house
default, because this wave spawns installers rather than git plumbing and an
undersized bound does not catch a hang -- it manufactures CI flake, which is
worse, since a flake gets re-run instead of investigated. install.test.cjs
records a real spawnSync ETIMEDOUT at a 60000ms cap on a loaded bench while
another lane passed the same commit in 12.7s, so full installs are bound at
120000ms against that recorded incident.

Also adds an auditable escape to the guard's timeout ceiling. The 600000ms
cap was set in #3143 from partial evidence, but fragment-single-edit-
propagation carries a documented, load-tested 900000ms bound on a run that
chains a full build plus eight generators -- the guard would have rejected a
correct timeout the moment that file left the allowlist. A value above the
ceiling is now permitted only with an inline allow-spawn-timeout-ceiling
marker carrying a non-empty reason. It raises the ceiling; it never waives
the requirement for a bound, which is asserted directly.

install-shared.cjs keeps its hand-rolled assert rather than routing through
throwIfFailed: its message embeds both streams, and throwIfFailed carries
only a trimmed stderr. The message now also names the outcome, so a bounded
timeout reads as such across its 38 importers instead of as
expected null to equal 0.

* test(#3145): extract class-norm timeouts and correct the build-hooks sizing

A pre-PR review found 52 copies of four class-norm timeout constants across
this wave. These are not per-suite fixture bindings -- they are shared facts
about how long a class of subprocess takes, derived from a recorded bench
incident. That norm already moved once (60000 to 120000 after a real
ETIMEDOUT), and 52 copies would have drifted the next time it moved.

Extracts tests/helpers/timeouts.cjs, where each norm is justified once, and
converts the copies. A site that genuinely differs -- a real tsc compile, or
regen:derived -- keeps its own local constant with its own justification.

Also corrects a misclassification: scripts/build-hooks.js was sized as a
build at 120000 in twelve places and 60000 in another, but it compiles and
bundles nothing. Its own header says no bundling needed; it copies pre-built
files and syntax-checks them with vm. Three different values bounded one
script; now there is one.

* test(#3145): fix red CI — lint self-match and a Windows chunk overrun

Two failures on PR 3176.

lint-allow-test-rule-refs read a RuleTester fixture as a real exemption. The
fixture exists to prove an unrelated marker does NOT suppress the rule, so it
carries that marker's literal text as test data. Split via concatenation, the
same idiom no-unbounded-spawn-allowlist.test.cjs already uses for its own
self-match problem. The explanatory comment needed the same treatment.

The Windows shard 3/3 chunk was killed at its 600000ms budget. Output stopped
seven minutes before the kill, so this was an overrun rather than a slow
chunk: regenDerivedPropagatesSingleFragmentEditWithNoSecondSourceSurface runs
regen:derived bounded at 900000ms, which is larger than the whole chunk
budget, so the chunk killer always fires first and it can never complete
there. Both the test and that bound predate this change; modifying the file
pulled it into the Windows targeted set and exposed it. Skipped on Windows
with the reason recorded; the Linux lanes cover it. The 900000 bound and its
ceiling marker are unchanged -- they are correct.

* test(#3145): refresh the stale test-timings cost table

The Windows shard was killed at its 600000ms per-chunk budget. run-tests.cjs
packs chunks by measured duration from tests/test-timings.json, and an
unknown file falls back to the table's median weight -- advisory by design,
but it silently underweights exactly the files that matter.

Four of the failing chunk's 22 files were absent from the table, including
the two heaviest: fragment-single-edit-propagation.install.test.cjs at 230s
(it runs regen:derived) and agent-fragments-emission.install.test.cjs at 79s.
Both were weighted as average, so the chunk's total weight read 53.68 against
a budget of 60 and the packer produced a single chunk.

Regenerated from a passing full-suite run, per the remedy the script itself
documents. 700 to 770 entries, 70 added, 0 dropped -- verified, since
gen-test-timings.cjs replaces the table wholesale rather than merging.

Proven against the real packer: the same 22 files now weigh 103.91 and split
into two chunks. No logic, budget, or timeout was changed; raising a budget
to make a red gate pass is not a fix.

6ad30f74b6 feat(#2584): Phase 3 — scheduler consumer + isolation adapters (#2635)
Final phase of #2584 (ADR-1239 Codex-binding amendment). execute-phase now negotiates dispatch.isolation and dispatches through the matching adapter, so a wave's independent plans run concurrently on six runtimes instead of one — with no runtime=== branch in the scheduler.

harness-worktree passes the host's declared isolation flag (claude, cursor); orchestrator-worktree creates the worktree via the Phase-2 verb and spawns the executor into it with the resolved argv/cwd (codex, opencode, kimi, kimi-code); none stays sequential. Undeclared/unknown/unresolvable isolation degrades to none — never an unisolated parallel run.

Fixes two shipped Phase-2 descriptors that per-host research found would fail at spawn: kimi lacked its headless flag (would launch the interactive TUI and hang the orchestrator), and kimi-code named a non-existent binary (Kimi Code installs as 'kimi'). Adds the worktree-path root confinement Phase 2 deferred here, and leading-dash guards on the resolver's prompt/cwd matching the existing git-argument guard.

now the sole canonical registration source. The existing marker-truncate

[agents.gsd-*] tables from prior installs, so updates converge to zero
duplicates without any new migration path.

`claude-*` ids. Codex/ChatGPT rejects those (400 "The 'sonnet' model is not supported
when using Codex with a ChatGPT account"), and since spawn_agent has no inline model
param, the model is baked into the .toml at install time — so the orchestrator could
not recover and fell back to the non-equivalent generic-agent workaround.

Translate a GSD tier alias through the Codex tier map (sonnet -> gpt-5.6-terra); drop
with a deduped warning any Anthropic-flavored value with no Codex mapping (fable) or a
`claude-*` id, so emission falls through to the runtime-aware resolver or Codex's
default. A final safety gate blocks an Anthropic-flavored model from the runtime-
resolver path too (runtime/target mismatch). Mirrors the Claude-side override guard
(#2041). Real Codex/OpenAI model ids in model_overrides still pass through verbatim
(#2256 preserved); runtime:"codex" tier resolution unchanged (#2517).

model-catalog.json to the GPT-5.6 family (gpt-5.6-sol/terra/luna),
advancing from the superseded GPT-5.4/5.5 generation.

Model IDs verified against OpenAI developer API docs:
- gpt-5.6-sol: flagship, /, reasoning xhigh
- gpt-5.6-terra: balanced, .50/, reasoning medium
- gpt-5.6-luna: fast/cheap, /, reasoning medium

Tier mapping is 1:1 (Sol↔flagship, Terra↔balanced, Luna↔fast),
so profile semantics are unchanged — only the underlying IDs advance.

Updates: catalog JSON, test assertions (catalog defaults), docs
(CONFIGURATION.md + zh-CN/pt-BR translations, workflow settings),
and changeset.

6e773d97df feat(#2088): migrate Codex onto the Embeddable Orchestration System (ADR-1239)
Drive Codex install/uninstall through the descriptor-driven Host-Integration
Interface (declarative embedding adapter → engine surface dispatch) and fold
every positive `runtime === 'codex'` / `isCodex` projection into descriptor-driven
`runtime.hostBehaviors`. Install/uninstall output stays byte-parity-gated
(tests/fixtures/golden-install-parity/codex.json); no other runtime changes.

Three Context7-verified upgrades, each with a test on the user-reachable surface:
- Skill root → canonical $HOME/.agents/skills via a skills-kind `home` override,
  with pre-move migration cleanup (stale ~/.codex/skills/gsd-* removed on install
  and uninstall; user content preserved). Fixes getGlobalSkillsBase, writeManifest,
  and the skill-manifest inventory to honor the override so --skills-root /
  sync-skills / the manifest report the real location.
- Six new hooks.json lifecycle events (PreToolUse, PermissionRequest, PreCompact,
  PostCompact, SubagentStop, UserPromptSubmit) shared by install + uninstall;
  extendedHookEvents reconciled [] -> the schema-valid wired subset.
- Explicit `[agents] max_depth = 1` in the managed config.toml block, pinning the
  negotiated dispatch.maxDepth:1 axis. validateCodexConfigSchema now permits a
  known-scalar-only bare `[agents]` AgentsToml table (still rejects [[agents]] and
  unknown-key break-forms, #2760); mergeCodexConfig preserves the user's own
  AgentsToml scalars (max_threads etc.) instead of dropping them.

--gemini now prints an explicit deprecation notice citing the 2026-06-18 sunset and redirects to --antigravity (no silent alias, per the issue's Hyrum's-Law rejection). Antigravity is preserved throughout: its GEMINI.md contextFileName, .gemini/antigravity config home, the shared convertGeminiToolName/claudeToGeminiTools tool vocabulary, and the 'gemini' hookEvents dialect it declares. GEMINI.md retargeted as Antigravity's context file.

feat-3593 cli-negative-universal) into a NEW shared repo-invariants.test.cjs; the other 18 as
singletons into their nearest module suite (model-resolver, codex-config, runtime-converters,
security, state-transition, worktree-safety, roadmap-parser, etc.). Verbatim block-scoped
describe wrappers; 334 subtests conserved 1:1.

Host-env pre-check (B2+B6): the 6 CLI folds into GSD_TEST_MODE-setting hosts (model-resolver/
codex-config/runtime-converters) are benign — each origin independently sets GSD_TEST_MODE=1
itself (idempotent), unlike the B6 real-install case.

Regenerates regression-name allowlist (222->213), ratchets file-count allowlist (state 17->16),
makes 7 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456; prunes stale ids).
Repoints 2 tests/ refs in docs/TESTING-SUITES.md. lint:ci green.

- Replace writeCodexSkillMetadataFiles / generateCodexSkillMetadataYaml with
  cleanupCodexSkillMetadataSidecars: Codex-only (if isCodex), removes stale
  managed gsd-*/agents/openai.yaml and prunes the now-empty agents/ dir.
- Preserve user-owned dirs (gsd-dev-preferences), non-empty agents/ dirs, and
  non-gsd dirs; lstat-guard against symlinked agents/ so a delete can never
  escape the skills tree; fail-open per directory.
- Codex relies on SKILL.md alone for /skills discovery.
- Update USER-GUIDE/FEATURES docs and rewrite the #774 emission tests into
  cleanup tests.

Scope: the sidecar duplicate only. The separate multi-root (~/.agents/skills
shared-skills) duplicate facet is a distinct concern, not addressed here.

The sandboxTier runtime-capability axis was cosmetic: declared and validated
on all 16 descriptors but read by nothing. The codex per-agent sandbox_mode
line was emitted unconditionally from the hardcoded CODEX_AGENT_SANDBOX map, so
the descriptor field drove no behaviour (ADR-857 audit finding F10; the
"rides along in 5e/5g" promise in ADR-1016 §8 never landed).

Make the axis load-bearing:
- resolveInstallPlan projects sandboxTier as a 7th InstallPlan axis and fails
  loud (throws) on a missing/invalid value rather than coercing to 'none'.
- installCodexConfig / generateCodexAgentToml gate sandbox_mode emission on
  sandboxTier !== 'none'.
- The per-agent CODEX_AGENT_SANDBOX map is kept: it is GSD agent policy, not a
  runtime-descriptor property (different layer). Full removal of that map is
  tracked under #1138 (phase-6 descriptor-residue removal).

For codex (sandboxTier === 'codex-agent-sandbox') the emitted TOML is
byte-identical to before; for 'none' runtimes sandbox_mode is omitted.
Adds leaf, projection, and installCodexConfig threading-seam regression tests;
updates the enh-1082 InstallPlan golden master with sandboxTier for all 16
runtimes. Confirmed hypothesis: schema-first vocabulary closure outran consumer
wiring, with no conformance gate to catch the orphaned axis.

Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the
on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary
(`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers
are unaffected.

Mechanical (bulk, ~90% of the diff):
- `git mv get-shit-done gsd-core`
- Swept path/identifier references across the repo via
  `perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead
  preserves the five legitimate slug variants that are NOT the directory:
  get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names).
- Build/manifest wiring: package.json (bin, files, coverage globs),
  tsconfig.build.json (outDir), ~86 .gitignore build-output entries,
  stryker.config.mjs, scan-ignore files, install.js path strings.
- Frozen (not rewritten): CHANGELOG.md history; translated docs
  (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/).

New logic (review here):
- src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper
  ADR-0008 installer migration. On upgrade it walks the legacy
  `~/.claude/get-shit-done/` tree, classifies each file via the prior install
  manifest, and emits remove-managed / backup-and-remove for managed files
  while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked
  root and symlinked entries; bounds-checks every path under configDir). The
  framework rolls back on install failure. Emptied dirs may remain (framework
  has no recursive dir-removal primitive) — documented.
- scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare
  `get-shit-done` directory token (split token to avoid self-match; case-
  insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists
  CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines).
  Wired into the lint-tests CI job.
- Restored scripts/lint-package-identity-drift.cjs detection regexes (the
  mechanical sweep had wrongly rewritten the old-name patterns it exists to
  detect) and marked them as intentional legacy references.
- TDD tests for the migration and the guard; do.md slash-command guard regex
  tightened so a `/gsd-core/bin` path segment is not mistaken for a command;
  changeset + docs/installer-migrations.md row added.

Breaking: the installed runtime path moves `~/.claude/get-shit-done/` ->
`~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed
files (preserving user files) on upgrade. Users with custom hooks/configs
hardcoding the old path must update them.

a28dcec981 chore(#597): replace count-based ratchet guards with AST lint + named-set allowlists (#603)
The windows-test-parity ratchet greps test source for fs.rmSync-without-
maxRetries (and six other Windows-portability anti-patterns), failing when an
integer offender COUNT exceeds a frozen baseline (rmSync: 95). A count ratchet
is a Goodhart metric: fixing one offender and adding another keeps the count
constant, so a new defect slips through green. Replace it — and every other
count ratchet in the repo — with a layered, masking-proof design.

Behavioral seam test
- tests/helpers-cleanup.test.cjs proves helpers.cleanup() carries the Windows
  EBUSY retry budget. cleanup() delegates retries to Node's fs.rmSync via
  maxRetries (it owns no loop), so the test asserts the option contract
  (recursive/force/maxRetries>0/retryDelay>0) + real-FS removal + the cwd-guard,
  rather than a loop that does not exist. The EBUSY risk is now tested ONCE at
  the helper, not approximated textually at every call site.

Write-time ESLint rule (AST-accurate, replaces the grep)
- eslint-rules/no-raw-rmsync-in-tests.cjs (error in tests/**/*.test.cjs) bans
  raw fs.rmSync, steering to cleanup(). Catches member, computed (fs['rmSync']),
  destructured and aliased forms; escape hatch is inline
  `// eslint-disable-next-line local/no-raw-rmsync-in-tests -- <reason>` only.
- Migrated 336 raw fs.rmSync teardown calls across ~116 test files to cleanup().
  ~18 genuinely load-bearing sites (mid-test SUT/fault-injection removals,
  error-swallowing or name-colliding local teardown helpers) keep the raw call
  with an inline eslint-disable + reason.

Shared anti-ratchet primitive
- scripts/lib/allowlist-ratchet.cjs:
  - assertWithinAllowlist: fails on NOVEL ids (new offender introduced) AND on
    STALE ids (a known offender was fixed but not pruned) — identity, not count,
    and a ratchet DOWN toward zero.
  - assertTightCeiling: a size/length budget whose ceiling must stay within a
    grace band of the high-water mark, so budgets may only tighten, never creep.

Ratchets converted onto the primitive
- windows-test-parity-guard.test.cjs: rmSync rule deleted (now ESLint-enforced);
  the remaining six patterns moved from integer baselines to named-set
  allowlists with ratchet-down.
- scripts/lint-test-file-count.{cjs,allowlist.json}: per-module integer counts →
  named filename sets (closes the swap-a-file-keep-the-count blind spot); a
  module dropping under cap now FAILS to force pruning its allowlist entry.
- enh-2790 skill-count `<= 63` → named skill allowlist (ratchets toward ~58).

Size budgets hardened (tighten-only)
- agent-size / workflow-size / feat-3039 help-tiered: ceilings lowered to the
  current high-water mark and an assertTightCeiling anti-creep check added per
  tier. Fixed external-contract limits (description ≤100 chars, agent ≤100 KB)
  are intentionally left as-is — they are not grandfathered creeping budgets.

No user-facing behavior change (tests + tooling only); no USER_FACING_PREFIXES
touched, so no changeset fragment is required.

win32 to write a .cmd shim alongside the .js hook file. cmd.exe executes .cmd files
natively via CreateProcess, bypassing the POSIX exec layer entirely. Non-Windows
paths (darwin, linux) are unchanged: they continue to use the node-runner command.

Also adds `gsd-check-update.cmd` to the codex-hooks-json managed-basename set so
reconcileCodexHooksJsonSessionStart correctly replaces stale node-runner entries on
reinstall.

1746f89ac7 fix(3566): narrow scope; update codex-config tests for canonical hooks key
Two follow-up adjustments after running the full suite:

1. Reverted the rewriteTomlKeyLines() change. The original
   `match.keyRaw || key` fallback respects user ownership of pre-existing
   legacy lines (#2760 defensive principle). My fix now applies the
   canonical-vs-legacy split at the INSERTION points only: fresh installs
   write `hooks = true`, but a pre-existing user-authored
   `codex_hooks = true` is preserved verbatim. Codex's own runtime
   legacy_key alias handles backward-compat at the Codex layer.

2. Updated 12 test cases in tests/codex-config.test.cjs that pinned the
   old fresh-write key. These assertions now expect canonical `hooks` for
   fresh-write scenarios; tests covering legacy-line preservation already
   pass against the narrowed fix without further edits.

Also updated bug-3566 regression-test cases for the legacy-preservation
path — they now verify that user-owned `codex_hooks` survives an install.

  `npm install --no-package-lock` resolves whatever satisfies semver at
  install time — CI today and CI in six months can install different
  transitive trees, defeating reproducibility.
- The lockfile has been part of every release on this repo (long
  history on main); removing it loses the npm audit / Dependabot
  target without compensating benefit.
- The CI workaround pattern (cache-dependency-path: sdk/package-lock.json
  + `npm install --no-package-lock`) papered over the symptom rather
  than fix the cause.

Also fix the changeset pr: from 3312 (issue) to 3316 (PR). CONTEXT.md
flags this exact failure mode as a recurring CodeRabbit finding.

PR #3002 fixed #2979 for settings.json-based managed JS hooks (Claude
Code, Gemini, Antigravity) by routing through buildHookCommand() →
resolveNodeRunner(), emitting the absolute Node binary path so hooks
resolve under GUI/minimal-PATH runtimes (/usr/bin:/bin:/usr/sbin:/sbin)
where nvm/Homebrew/Volta-installed node is not on PATH.

The Codex install path bypassed both helpers — line 7935 of bin/install.js
wrote `command = "node ${path}"` directly into config.toml. So Codex
SessionStart hook still failed with exit 127 ("node: command not found")
under the same minimal-PATH conditions PR #3002 was meant to close.

Fix:
- Add buildCodexHookBlock(targetDir, { absoluteRunner, eol }) — a pure
  helper that emits the toml hook block with the absolute runner. Returns
  null when absoluteRunner is null so the caller skips registration with
  a warning instead of writing a broken bare-node hook.
- Add rewriteLegacyCodexHookBlock(content, absoluteRunner) — mirror of
  rewriteLegacyManagedNodeHookCommands for the toml surface, so
  reinstall migrates a 1.39.x bare-node config.toml to the absolute form.
  Uses basename equality (CODEX_MANAGED_HOOK_BASENAMES set) so user-
  authored bare-node hooks are left alone.
- Replace the inline string-concat at line 7935 with a call to the new
  helper, threaded with the detected line ending so CRLF files stay CRLF.
- On the codex reinstall path, call rewriteLegacyCodexHookBlock first so
  existing bare-node entries get migrated before the new entry is added.

Tests:
- bug-3017-codex-hook-absolute-node.test.cjs (9 tests, all typed-IR):
  - buildCodexHookBlock emits absolute runner, parses to expected fields
  - returns null on missing runner (caller skips)
  - integrates with resolveNodeRunner() in the live process
  - rewriteLegacyCodexHookBlock migrates managed bare-node entries
  - leaves user-authored bare-node hooks alone (basename allowlist)
  - leaves entries with absolute runner unchanged (idempotent)
  - returns content unchanged when absoluteRunner is null
- codex-config.test.cjs e2e expectation updated to match new shape:
  parsed.hooks.SessionStart[0].hooks[0].command now equals
  '"<process.execPath>" "<hookPath>"' instead of 'node <hookPath>'.

Verification:
- 9/9 pass on the new regression test
- 179/179 pass across all codex-touching test files
- 6767/6767 pass on full suite, lint-no-source-grep clean
- Adheres to typed-IR / CONTRIBUTING.md "Prohibited: Raw Text Matching":
  parseCodexHookBlock returns a typed record; assertions are on
  structured fields (runner, hookPath, type, hasMarker), not stdout regex.

CodeRabbit on PR #3022 (3 findings, 2 actionable + 1 nitpick):

1. .changeset/codex-bare-node-fix.md:3 — replace `pr: TBD` with

2. tests/bug-3017-codex-hook-absolute-node.test.cjs:81-146 — the test
   asserted `parsed.runner !== 'node'` and `parsed.runner.includes('/node')`,
   which would false-positive on any absolute path containing '/node'
   (e.g. /Users/x/notnode/foo). Tightened to compare against the EXACT
   absolute path supplied by the caller (after stripping toml + JSON
   escape layers via a new unescapeRunner() helper). The live-process
   integration test now compares against process.execPath exactly. The

3. Nitpick (skipped): use repository's TOML parser for parsing instead
   of bespoke regex. The hand-rolled parser is small, scoped, and
   fully tested by these structural assertions; pulling in a TOML lib
   for tests would create a circular dependency on the SUT (the
   installer's own parser). Leaving as-is.

Verification: 9/9 pass on regression test, 6767/6767 full suite, lint clean.

      pass 1 collects vars bound from readFileSync, pass 2 finds any
      <var>.<includes|startsWith|endsWith|match|search>( on those vars.

      assert.ok(<expr>.match(...)) which escapes the assert.match rule.
    - VIOLATION enum exposes stable codes for tests to assert on.

  scripts/lint-no-source-grep.cjs
    - Wires the new detectors into the existing per-file check; one
      additional violation row per file with the first 3 sample tokens.

  tests/bug-2982-lint-var-binding.test.cjs
    - 13 tests, all assertions on typed VIOLATION enum / structured
      records. Covers all 5 text-match methods, multi-var, no-bind,
      string literal (must NOT trigger), wrapped assert.ok(.match),
      and assert.match (must NOT double-flag).

Migration backlog (#2974 expanded scope):

  - 42 files annotated `// allow-test-rule: source-text-is-the-product`
    (legitimate — they read .md/.json/.yml files whose deployed text
    IS the product)
  - 3 files annotated `// allow-test-rule: pending-migration-to-typed-ir [#2974]`
    (read .cjs/.js source — clear migration debt)
  - 95 files annotated `pending-migration-to-typed-ir [#2974]` with
    `Per-file review may reclassify as source-text-is-the-product
    during migration` (mixed — manual review under #2974)

After this lands the lint reports 0 violations on main; new
violations in PRs surface immediately.

Previous GSD versions wrote the flat [[hooks]] + event = "SessionStart"
form (#2637) or a single-block [[hooks.SessionStart]] without the nested
.hooks sub-table (#2760). Both are rejected by Codex 0.124.0+ at launch.

Changes:

bin/install.js
  - Hook block emission now always writes the two-level nested AoT form.
  - migrateCodexHooksMapFormat extended to also migrate flat [[hooks]]
    array-of-tables entries (event = "..." key → [[hooks.<EVENT>]] form).
    Flat [[hooks]] and [[hooks.<EVENT>]] are mutually exclusive TOML types;
    any pre-existing flat entries must be promoted before GSD appends its
    own namespaced hooks.
  - Migrated flat AoT blocks are inserted BEFORE the GSD marker so they
    stay in the "user" portion of the file and survive stripGsdFromCodexConfig.
  - stripCodexGsd* regexes cover all four historical block shapes.
  - validateCodexConfigSchema no longer rejects flat [[hooks]] at the root
    level (removing the false-positive that blocked install when users had
    their own AfterCommand hooks). The validator still enforces the nested
    [[hooks.<EVENT>.hooks]] shape for entries that have a .hooks sub-table.

tests/
  - bug-2760-codex-install-defensive.test.cjs: 29/29 passing.
    Added 5 new regression cases for fresh install, upgrade from each
    legacy shape, idempotent reinstall, and user hook preservation.
  - codex-config.test.cjs: 106/106 passing.
    All migration tests updated to assert [[hooks.<TYPE>.hooks]] sub-table
    (command now in handler level, not event-entry level).
    New tests: flat [[hooks]] migration (SessionStart, AfterCommand),
    install+uninstall preserves non-GSD AfterCommand hook.

  Four tests cover the new stale AoT migration path: single-entry promotion,

  to the CRLF migration test (the one miss from CR round 2).

  With stale AoT entries now migrated, the entry.command fallbacks in

  level produces an explicit test failure rather than silently collecting them.

Three defects, three defensive fixes shipped together. Issue reporter
never returned with the requested diagnostic backup, but four additional
users have since confirmed the same Codex breakage and ZakAnun confirmed
manual cleanup is the only working workaround — defensive triple ships
without the original backup grep, justified by the corroborating reports.

Fix 1 (defect 3 — confirmed real). The Codex hooks emit path always
appended a top-level `[[hooks]]` AoT block, which collides with users
who already use the namespaced AoT form `[[hooks.SessionStart]]`. New
helper `hasUserNamespacedAotHooks()` detects the user's preferred shape
on parse and the install emits the GSD-managed hook in that same shape
when present. Default for fresh configs stays at top-level `[[hooks]]`
so status-quo behavior is preserved.

Fix 2 (defects 1+2 — defensive). `stripLeakedGsdCodexSections()` (the
install-time stripper) now always purges bare `[agents]` single-bracket
tables and `[[agents]]` sequence tables regardless of GSD marker
presence — both forms are invalid in current Codex schema and produce
"invalid type: ..., expected struct AgentsToml". Previously gated on
GSD-name lookup which missed marker-stripped configs and third-party
authored entries. The uninstall-time stripper (`stripCodexGsdAgentSections`)
keeps its old conservative behavior so user-authored entries survive
uninstall.

Fix 3 (defensive). Post-write schema validation parses the bytes about
to be committed and asserts no bare `[agents]`, no `[[agents]]`, and no
bare `[hooks.<Event>]` tables remain. On failure the install restores
the pre-install backup of config.toml and aborts loudly so the user is
never left with a Codex CLI that refuses to load. Pre-install snapshot
is captured before installCodexConfig runs (not after) so restore
returns the file to its true pre-GSD state.

Tests added (10 new, 1 updated):
  - bug-2760-codex-install-defensive.test.cjs (10 new tests across 4
    describes: hooks AoT preservation, strip robustness for both
    [agents] and [[agents]] without marker, schema validator behavior,
    abort+restore via test seam)
  - codex-config.test.cjs "case 2 ..." updated to reflect new defensive
    bare-[agents] purge

Full suite: 5747 pass / 0 fail.

[hooks.TYPE] sections, while the GSD-managed Codex install emitted
`event = "SessionStart"` — same target [[hooks]] schema, two different
field names. Codex currently tolerates both via permissive parsing, but
the moment one path tightens this becomes a silent #2760-class regression.

Normalize both call sites on `event` (the existing GSD-managed
convention). Update migration emit, docstring, and existing migration
assertions to match. Add a parity regression test that drives both code
paths and asserts the [[hooks]] field key is identical.

* test(#2153): fix test isolation by building hooks/dist on demand

The "Codex install copies hook file (#2153)" regression depends on
hooks/dist/ being populated, but that directory is gitignored and only
built by `npm run build:hooks`. The npm pretest chain runs `build:sdk`
but not `build:hooks`, so when this file is run in isolation
(`node --test tests/codex-config.test.cjs`) the hook copy step skips
silently and the regression test fails on a stale-environment artifact
rather than a real bug.

Add a top-level before() hook that runs scripts/build-hooks.js when
hooks/dist/ is missing or empty. Matches the pattern already used by
bug-1834-sh-hooks-installed and other install integration tests, so the
suite passes regardless of runner ordering or which tests are targeted.

* fix(#2760): structural TOML validation, atomic writes, and behavioral test rewrites

Addresses CodeRabbit review on PR #2785 plus source-grep violations the
maintainer flagged in the regression test.

Fix 1 (CR 3149606220) — validateCodexConfigSchema now parses the TOML into
a structured object first via the new parseTomlToObject helper, then
runs schema-shape checks against both the parsed structure and the table
section headers. Malformed TOML with valid-looking headers no longer
slips past validation.

Fix 2 (CR 3149606224) — Replaced the four source-grep assertions in
tests/bug-2760-codex-install-defensive.test.cjs (lines 109, 125, 169,
201) with structural assertions against the parsed TOML object via the
exported parseTomlToObject helper. Tests now verify behavior (the file
parses and contains the expected structure) instead of literal byte
patterns. Robust to formatting changes — exactly what the regex-loosening
suggestion was reaching for, done correctly. Confirmed clean by
`npm run lint:tests` (0 violations).

Fix 3 (CR 3149606234) — The describe block that mutates
installModule.__codexSchemaValidator now runs with concurrency: false
so the test seam mutation cannot leak into sibling suites that also
call runCodexInstall.

Fix 4 (CR outside-diff) — Approach (b): atomic temp-file + renameSync.
Added atomicWriteFileSync helper used by mergeCodexConfig and the final
hooks-write. A mid-write failure leaves the .tmp-<pid>-<n> sibling
behind (cleaned up immediately) and never truncates the original
config.toml. Paired with try/catch wrapping around the entire
post-snapshot mutation sequence so any unexpected throw also triggers
restoreCodexSnapshot. Two layers of defense: atomic write prevents the
corruption window, snapshot restore handles non-atomic write paths.

Added behavioral test for fix 4: stubs fs.renameSync to throw on the
configPath rename, asserts the on-disk bytes match the pre-install
snapshot byte-for-byte, asserts the parsed structure is still the
user's [model] section (no half-written GSD agents block), and asserts
no stray .tmp-* files remain. Marked concurrency: false because it
monkey-patches a global.

Test results: 5749/5749 pass, 0 fail. lint:tests clean.

* test(#2760): TOML-parse based assertions for bare-agents purge and hook-field parity (CodeRabbit follow-up)

CR4 finding 1 (MAJOR) — Write failures silently succeeded. The inner catch
around atomicWriteFileSync restored the snapshot then re-threw, but the outer
catch only matched 'post-write Codex schema validation failed' and downgraded
everything else to a warn-and-continue. Install finished with "Done!" while
Codex had no GSD agents configured. Fix: wrap writeErr with a `post-write
Codex install failed:` prefix and broaden the outer guard to `.startsWith(
'post-write')` so both schema-validation and write failures abort install.

CR4 finding 2 (MAJOR) — Legacy flat [[hooks]] block prevented namespaced AoT
upgrade. The `!configContent.includes('gsd-check-update')` guard short-
circuited the new namespaced emit when an existing install had the legacy
flat [[hooks]] block, leaving users stuck in the mixed layout this fix is
designed to eliminate. Fix: strip ALL existing managed gsd-check-update
hook blocks (top-level [[hooks]] AND namespaced [[hooks.SessionStart]])
BEFORE evaluating the includes guard, so every install converges on the
right shape regardless of prior state.

CR4 finding 3 (MAJOR) — Homegrown TOML parser silently accepted malformed
input. parseTomlValue happily consumed the `0` prefix of `timeout = 0.5`
and parseTomlToObject did not verify the full RHS was consumed, so
`key = "x" junk` and date/time literals slipped through. Per CONTRIBUTING
("No external dependencies in core"), option (b) was chosen over adding
@iarna/toml: (a) parseTomlValue rejects any integer immediately followed
by `.`, `e`, `E`, `:`, `-`, `T`, or `Z` (floats / dates / times); (b)
parseTomlToObject scans from parsed.end to the next newline and throws
`trailing bytes after value` if anything other than whitespace + optional
`# comment` is present.

* test(#2760): add CR4 regression tests + scope GSD_TEST_MODE + rename rename-fault test

CR4 finding 5 (NIT) — GSD_TEST_MODE leak. Saved previous value, set '1' for
the require, then restored (delete if undefined). No more test-only env var
leaking to siblings in the same node process.

CR4 finding 4 (NIT) — Renamed the existing fix-4 test from 'fs.writeFileSync'
to 'fs.renameSync' (the only call actually faulted) and added a sibling test
that stubs fs.writeFileSync to throw on the .tmp- target — exercising the
pre-rename branch of atomicWriteFileSync that was previously untested. Both
serialize via concurrency: false on the existing describe block.

CR4 finding 1 (MAJOR test) — New behavioral test asserts install throws with
a `post-write Codex install failed` message AND never prints "Done!" when
the hook-block atomic rename fails. Captures stdout via console.log stub,
asserts byte equality of restored snapshot. Faults only the rename whose
temp source contains gsd-check-update so earlier mergeCodexConfig writes
are not collateral damage.

CR4 finding 2 (MAJOR test) — New TOML-parsed behavioral test for the
legacy-hook upgrade path: pre-install has [[hooks.SessionStart]] (user) +
legacy flat [[hooks]] managed gsd-check-update entry; post-install must
have hooks.SessionStart as Array-of-tables with both user hook and GSD
entry, and no top-level [[hooks]] AoT remaining. Also asserts exactly one
gsd-check-update entry (no duplicates).

CR4 finding 3 (MAJOR test) — parseTomlToObject regression suite: rejects
floats (timeout = 0.5), dates (created = 1979-05-27), trailing garbage
(key = "x" junk), and accepts trailing whitespace + # comment.

* fix(#2760): CR5 — pre-write fatal, TOML duplicate-key/header rejection, namespaced AoT migration

Address all five CodeRabbit round-5 findings on PR #2785:

Finding 1 (MAJOR) — Pre-write failures in the Codex hook configuration
catch (around bin/install.js:7002) used to fall through to console.warn
even though restoreCodexSnapshot() had already run. This produced "Done!"
output with no Codex hooks configured. Now wraps the original error with
a "(pre-write)" prefix and rethrows so install aborts loudly. Same defect
class as CR4 finding 1, different layer.

Finding 2 (MAJOR) — parseTomlToObject silently reused existing tables and
overwrote duplicate keys. Real TOML 1.0 rejects:
  - duplicate scalar key in same table ([a]\nx=1\nx=2)
  - re-declared [a] header (two [a] sections)
  - [[arr]] then [arr] for same path (shape mismatch)
Tracks pathShape, declaredHeaders, and per-table-instance key sets;
throws "duplicate or shape-mismatched table header at <path>" or
"duplicate key <name> in <path>".

Finding 3 (MAJOR) — migrateCodexHooksMapFormat used to emit flat
[[hooks]]\nevent="<TYPE>", which produced mixed flat+namespaced layouts
when the user already had [[hooks.<OTHER>]] entries. Now emits
[[hooks.<TYPE>]] directly (the namespace IS the event); managed-emit
detector hasUserNamespacedAotHooks fires correctly so the install
converges on a single namespaced layout regardless of pre-existing state.

Finding 4 (NIT) — tests/bug-2760-codex-install-defensive.test.cjs
rename-failure test tightened from "throw OR warn acceptable" to
assert.equal(threw, true), locking the contract Finding 1 establishes.

Finding 5 (NIT) — bug-2760 test suite snapshots and restores fs.renameSync
defensively in beforeEach/afterEach (symmetric with fs.writeFileSync),
removing the fragile per-test try/finally. Second test in the same
suite cleaned up to drop its try/finally.

Updates tests/codex-config.test.cjs to assert the new namespaced AoT
migration shape via parseTomlToObject (no source-grep). Existing field-
parity test reframed as shape-parity since both paths now emit
namespaced.

Tests: 5764 pass (+8 new). lint:tests: 0 violations.

* docs(#2760): add CHANGELOG entry for Codex install defensive triple

Adds the [Unreleased] Fixed entry for the Codex install fix landed in this
PR — defensive strip of legacy [agents]/[[agents]] blocks, namespaced AoT
hook detection across all events, atomic write + rollback, strict TOML
validation rejecting duplicate keys/repeated headers/trailing bytes.

d5cd64dde5 fix(#2637): migrate legacy Codex [hooks] map format to [[hooks]] array on install (#2747)
Codex 0.124.0 changed the required config.toml hooks format from the old
map-style ([hooks.shell]) to array-of-tables ([[hooks]]). Old GSD installs
that wrote the legacy format now cause a startup parse error on upgrade.

Add migrateCodexHooksMapFormat() which detects non-array [hooks] and
[hooks.TYPE] sections and rewrites them to [[hooks]] entries with an
injected type = "TYPE" key. The migration runs at the start of every Codex
install so affected configs self-heal on the next `gsd install --codex`.

Codex ≥0.116 rejects `[agents.<name>]` map tables with `invalid type:
map, expected a sequence`. Switch generateCodexConfigBlock to emit
`[[agents]]` array-of-tables with an explicit `name` field per entry.

Strip + merge paths now self-heal on reinstall — both the legacy
`[agents.gsd-*]` map shape (pre-#2645 configs) and the new
`[[agents]]` with `name = "gsd-*"` shape are recognized and replaced,
while user-authored `[[agents]]` entries are preserved.

Fixes #2645

* fix(#2645): use TOML-aware parser to strip managed [[agents]] sections

CodeRabbit flagged that the prior regex-based stripper for [[agents]]
array-of-tables only matched headers at column 0 and stopped at any line
beginning with `[`. An indented [[agents]] header would not terminate the
preceding match, so a managed `gsd-*` block could absorb a following
user-authored agent and silently delete it.

Replace the ad-hoc regex with the existing TOML-aware section parser
(getTomlTableSections + removeContentRanges) so section boundaries are
authoritative regardless of indentation. Same logic applies to legacy
[agents.gsd-*] map sections.

Add a comprehensive mixed-shape test covering multiple GSD entries (both
legacy map and new array-of-tables, double- and single-quoted names)
interleaved with multiple user-authored agents in both shapes — verifies
all GSD entries are stripped and every user entry is preserved.
