pattern would silently overwrite an existing lock file and break the
locking guarantee.

* chore(coderabbit): disable ESLint tool (repo uses custom lint scripts)

CodeRabbit's review surface emits a "skipped: no ESLint configuration"
warning because the repo doesn't ship ESLint config. The repo
intentionally does not use ESLint — it ships its own targeted lint
scripts (scripts/lint-no-source-grep.cjs, npm run lint:tests) that
enforce repo-specific test-quality invariants. Adding ESLint config
purely to satisfy CR would add an external dependency
(CONTRIBUTING.md: "No external dependencies in core") and overlap
with the existing custom lint surface.

Disable the ESLint tool in CR's tools config so the skip warning
stops appearing on every PR.

ab5ad6c8bc fix(#2757): unquoted truths with colons crash annotate-dependencies (#2759)
parseMustHavesBlock dispatched on `includes(':')` to detect key-value pairs,
but unquoted YAML strings like `GET /foo/:id resolves...` and
`Class::Method is idempotent` also contain colons. When the KV regex failed
to match, `current` was left as `{}` (the empty object initialized before
the branch), which then caused `t.trim()` in roadmap.cjs to throw
`TypeError: t.trim is not a function`.

Two fixes:
- frontmatter.cjs: tighten the KV regex to require at least one space after
  the colon (`\s+` instead of `\s*`), matching YAML convention. When the
  regex still fails to match, fall back to treating the item as a plain
  string instead of leaving `current` as `{}`.
- roadmap.cjs: add `typeof t !== 'string'` guard before `.trim()` as a
  cheap safety net against any future parser anomaly.

1405728292 fix: parseMustHavesBlock quoted strings + gsd state complete-phase (#2744)
* fix: parseMustHavesBlock quoted strings + gsd state complete-phase

Bug #2734: parseMustHavesBlock dropped quoted truths containing ':' because
fully-quoted strings like `"App-side UUIDv4: generated locally"` fell into
the kv-parse branch, the regex failed (value starts with '"'), and current
stayed as empty {}. Fix: detect fully-quoted strings before the ':' check
and extract them directly. Two regression tests added to frontmatter.test.cjs.

Bug #2735: `gsd state complete-phase` subcommand was missing — unknown
subcommands fell through to cmdStateLoad. Added cmdStateCompletePhase to
state.cjs (updates Status, Last Activity, and Current Position to COMPLETE),
exported it, and wired it into the case 'state': dispatch in gsd-tools.cjs.

cad40fff8b fix(core): extend atomicWriteFileSync to milestone, phase, and frontmatter (#1972)
Replace 11 fs.writeFileSync calls with atomicWriteFileSync in three
files that write to .planning/ artifacts (ROADMAP.md, REQUIREMENTS.md,
MILESTONES.md, and frontmatter updates). This prevents partial writes
from corrupting planning files on crash or power loss.

Skipped low-risk writes: .gitkeep (empty files) and archive directory
writes (new files, not read-modify-write).

Files changed:
- milestone.cjs: 5 sites (REQUIREMENTS.md, MILESTONES.md)
- phase.cjs: 5 sites (ROADMAP.md, REQUIREMENTS.md)
- frontmatter.cjs: 2 sites (arbitrary .planning/ files)

Closes #1972

fa57a14ec7 fix: resolve REG-04 — frontmatter inline array parser now respects quoted commas (#1695)
Fixes #1694

The inline array parser used .split(',') which ignored quote boundaries,
splitting "a, b" into two items. Replaced with a quote-aware splitter
that tracks single/double quote state.

Updated REG-04 test to assert correct behavior and added coverage for
single-quoted and mixed-quote inline arrays.

d4859220e2 fix(core): add concurrency safety and atomic state writes
Wire withPlanningLock into all ROADMAP.md write paths (phase add/insert/complete/remove, roadmap update-plan-progress) to prevent concurrent corruption when parallel agents modify planning files.

Extract acquireStateLock/releaseStateLock from writeStateMd and add readModifyWriteStateMd helper that holds the lock across the entire read-modify-write cycle, preventing lost updates.

Replace O(n^2) normalizeMd fence detection with single-pass O(n) pre-computed fence state array.

Warn on must_haves parse failure and stateReplaceFieldWithFallback field miss.

1d97626729 fix: parseMustHavesBlock now handles any YAML indentation width
The parser hardcoded 4/6/8/10-space indent levels for must_haves
sub-blocks, but standard YAML uses 2-space indentation. This caused
"No must_haves.key_links found in frontmatter" for valid plan files.

The fix dynamically detects the actual indent of must_haves: and its
sub-blocks instead of assuming fixed column positions.

62db008570 security: add prompt injection guards, path traversal prevention, and input validation
Defense-in-depth security hardening for a codebase where markdown files become
LLM system prompts. Adds centralized security module, PreToolUse hook for
injection detection, and CI-ready codebase scan.

New files:
- security.cjs: path traversal prevention, prompt injection scanner/sanitizer,
  safe JSON parsing, field name validation, shell arg validation
- gsd-prompt-guard.js: PreToolUse hook scans .planning/ writes for injection
- security.test.cjs: 62 unit tests for all security functions
- prompt-injection-scan.test.cjs: CI scan of all agent/workflow/command files

Hardened code paths:
- readTextArgOrFile: path traversal guard (--prd, --text-file)
- cmdStateUpdate/Patch: field name validation prevents regex injection
- cmdCommit: sanitizeForPrompt strips invisible chars from commit messages
- gsd-tools --fields: safeJsonParse wraps unprotected JSON.parse
- cmdFrontmatterGet/Set: null byte rejection
- cmdVerifyPathExists: null byte rejection
- install.js: registers prompt guard hook, updates uninstaller

7156f02ed5 fix: semver 3+ segment parsing and CRLF frontmatter corruption recovery
- fix(core): getMilestoneInfo() version regex `\d+\.\d+` only matched
  2-segment versions (v1.2). Changed to `\d+(?:\.\d+)+` to support
  3+ segments (v1.2.1, v2.0.1). Same fix in roadmap.cjs milestone
  extraction pattern.

- fix(state): stripFrontmatter() used `^---\n` (LF-only) which failed
  to strip CRLF frontmatter blocks. When STATE.md had dual frontmatter
  blocks from prior CRLF corruption, each writeStateMd() call preserved
  the stale block and prepended a new wrong one. Now handles CRLF and
  strips all stacked frontmatter blocks.

- fix(frontmatter): extractFrontmatter() always used the first ---
  block. When dual blocks exist from corruption, the first is stale.
  Now uses the last block (most recent sync).

  Windows where all plans reported as wave 1 (#1085)

- fix(hooks): remove duplicate const cwd declaration in
  gsd-context-monitor.js that caused SyntaxError on every PostToolUse
  invocation (#1091, #1092, #1094)

- feat(state): add 'state begin-phase' command that updates STATUS,
  Last Activity, Current focus, Current Position, and plan counts
  when a new phase starts executing (#1102, #1103, #1104)

- docs(workflow): add state begin-phase call to execute-phase workflow
  validate_phase step so STATE.md is current from the start

460f92e727 feat: normalize generated markdown for markdownlint compliance (#1112)
Add normalizeMd() utility that fixes common markdownlint violations
in GSD-generated .planning/ files, improving IDE readability:

  MD022 — Blank lines around headings
  MD031 — Blank lines around fenced code blocks
  MD032 — Blank lines around lists
  MD012 — No multiple consecutive blank lines (collapsed to max 2)
  MD047 — Files end with a single newline

Applied at key write points:
  - state.cjs: writeStateMd() normalizes STATE.md on every write
  - frontmatter.cjs: frontmatter set/merge commands normalize output
  - template.cjs: template fill normalizes generated plan/summary files
  - milestone.cjs: MILESTONES.md archive writes normalized

Includes 14 new tests covering all rules, edge cases (CRLF, frontmatter,
ordered lists, code blocks, real-world STATE.md patterns).

c67ab759a7 refactor: split gsd-tools.cjs into 11 domain modules under bin/lib/
Break the 5324-line monolith into focused modules:
- core.cjs: shared utilities, constants, internal helpers
- frontmatter.cjs: YAML frontmatter parsing/serialization/CRUD
- state.cjs: STATE.md operations + progression engine
- phase.cjs: phase CRUD, query, and lifecycle
- roadmap.cjs: roadmap parsing and updates
- verify.cjs: verification suite + consistency/health validation
- config.cjs: config ensure/set/get
- template.cjs: template selection and fill
- milestone.cjs: milestone + requirements lifecycle
- commands.cjs: standalone utility commands
- init.cjs: compound init commands for workflow bootstrapping

gsd-tools.cjs is now a thin CLI router (~550 lines including
JSDoc) that imports from lib/ modules. All 81 tests pass.
Also updates package.json test script to point to tests/.
