636ec92107 refactor(#3185): phase enumeration has one owner and a decidable scope (#3222)
* test(#3185): failing-first phase-enumeration single-owner suite

Covers the enumeration rows with direct code evidence: 999.* backlog dirs
listed by progress/stats, the phase-0 sentinel divergence, the #1324
letter-prefixed-decimal negative space, and the destructive-path find —
cmdPhasesClear carries a fifth sentinel copy (/^999(?:\.|$)/) that excludes
999 but not 0, so a 0-* directory roadmap.analyze preserves is deleted there.

Also covers the pass-all degrade, which is where the defect actually lives:
when the milestone window declares no phases the filter becomes a literal
() => true and its heading-side sentinel exclusion is unreachable. A fixture
carrying phase headings keeps the filter active and never reaches that path.

Named for the derivation, not a module: the suite drives commands, phase,
milestone, workstream-inventory and state, and both the phase and
phase-locator buckets are already at the per-module test-file cap.

Committed alone so the remote runner records the failure before the fix.

Adds phase-locator.cts::listMilestonePhaseDirs as the single canonical owner
of "which phase directories belong to the current milestone". It applies the
milestone window AND the sentinel filter and returns a ScopedResult, so a
caller can tell a genuinely-empty milestone from an enumeration that could
not be scoped.

The sentinel test now runs against DIRECTORY NAMES and is unconditional.
getMilestonePhaseFilter excludes sentinels from its ROADMAP heading set, but
degrades to a literal () => true pass-all predicate when that set is empty --
at which point the heading set is never consulted and its sentinel exclusion
is unreachable exactly when it is needed. That degrade is the #3167 path, and
it is why stats already used the filter and still listed backlog directories.
The narrowing is sentinel-only: pass-all stays over-inclusive otherwise.

Sentinel copies deleted, canonical isSentinelPhaseId adopted:
  - cmdRoadmapAnalyze's local closure (parseInt === 0 || === 999), 2 call sites
  - cmdPhasesClear's /^999(?:\.|$)/ -- the DESTRUCTIVE path, which excluded
    999 but not 0, so a 0-* directory roadmap.analyze preserves was deleted

cmdStats also seeded rows from ROADMAP headings with no sentinel filter, so a
999 heading produced a row with no directory; that seed is filtered now.

cmdPhasesList routes only its ENUMERATION. --phase lookup searches the
physical set (scoping it would report an out-of-window phase as not found) and
--include-archived still merges archived dirs (they are by definition from
other milestones). Both exempt by documented reason, never a file allowlist.

Fixed inline, found while building: isDirInMilestone could not match a #1324
letter-prefixed-decimal directory (P0.0-foundation) to its own Phase P0.0
heading, so stats reported the phase with plans: 0 while its directory held
plan files. Defers to phase-id's extractPhaseToken rather than widening a
fourth bespoke regex; additive, so it can only admit directories.

* refactor(#3185): route the last two enumeration re-derivations

workstream-inventory countRoadmapPhases counted every `Phase` heading across
the whole ROADMAP -- no window, no sentinel filter -- so it counted 999.*
backlog and Phase 0 and spanned every milestone the document ever had. Its own
caller already resolved a currentVersion and passed it to getMilestonePhaseFilter
elsewhere in the same file; this was the sibling copy that never got the fix.

state.cts phaseInventoryProvider enumerated phase dirs with its own
/^(\d+)-(.+)$/ convention regex and neither filter, so a rebuilt STATE.md
inventory carried backlog and sentinel directories as current-milestone phases.
A non-COMPLETE enumeration scope now throws to the outer catch as a real scan
failure rather than reporting a confident undercount, mirroring the per-phase
scanPhasePlans contract beside it.

* refactor(#3185): consolidate 23 sentinel re-derivations onto one predicate

The whole-repo drift guard (ADR-3180 Decision 4a, no file allowlist) found the
sentinel rule re-implemented 23 times across 8 modules, in three regex variants
plus four integer-comparison forms. Most tested 999 only, so Phase 0 slipped
through them while roadmap.analyze and the engine-wide convention (#1580) both
treat 0 and 999 alike. That disagreement is the defect class this epic removes.

All 23 now call phase-id's isSentinelPhaseId (SENTINEL_RANGES [0,999]). Sites:
init recommended-actions and backlog counts, milestone phase scan, the
phase-lifecycle progress table, phase.cts used-number collection and the four
renumber-on-remove guards, roadmap-parser's heading and bullet milestone
counts, roadmap get-phase fallbacks, and state's heading denominator.

Excluding Phase 0 at these sites is a deliberate behavior change and the point
of the consolidation — several carried comments already saying 0 should be
excluded while the literal beside them caught only 999.

Adds scripts/lint-phase-enumeration-drift.cjs, wired into lint:ci. It scans the
whole src/ tree with no file allowlist and reports both shapes: an independent
phases-dir enumeration, and an independent sentinel literal. Exemptions are
function-scoped with a written reason. The guard is comment-aware — its first
pass flagged JSDoc and a comment documenting that the code below uses the
canonical owner, which would have trained readers to exempt prose.

Per-site triage of the 31 remaining whole-repo guard hits, applying the rule
generalized from #3183's Amendment 1: a LOOKUP, DIAGNOSTIC, ARCHIVAL or
MUTATION pass wants the physical set; only "which phases belong to this
milestone" wants the scoped set.

Routed (10): init new-milestone phase_dir_count, init milestone-op fallback
count, init manager, init progress, milestone complete stats/dry-run/archive
move, phase complete's next-phase scan, state update-progress, state
frontmatter stats, and uat audit's active set.

Exempt with a written function-scoped reason (never a file allowlist): the
audit/UAT/verification sweeps that deliberately scan every directory to report
gaps, phase create/insert/rename/renumber mutations, single-phase lookups,
roadmap-upgrade's cross-milestone migration, cmdPhasesClear's whole-tree
destructive pass, and the reads that list a phase dir's FILES rather than
enumerating the phases dir at all.

Latent defects fixed by the routing: sentinel directories leaked into
cmdInitNewMilestone's phase_dir_count, cmdMilestoneComplete's stats, dry-run
AND ARCHIVE MOVE, cmdStateUpdateProgress, buildStateFrontmatter and
cmdAuditUat's active set — every one of those hand-rolled an isDirInMilestone
filter with no sentinel exclusion, so `milestone complete` was archiving
backlog directories.

scripts/lint-phase-enumeration-drift.cjs now reports 0 re-derivations and
npm run lint:ci is green.

Amendment 3 records: the SCOPE contract held unchanged; the declared deviation
from Decision 1's provisional signature (the window needs cwd/ws, which the
locked roadmapContent parameter cannot supply); the copy count being a lower
bound for the third consecutive phase (4 scoped vs 54 found); the load-bearing
finding that the sentinel exclusion sat on the heading set and was unreachable
under the pass-all degrade; the two destructive-path defects; and the
generalized exemption rule.

Review + remote runner findings, all fixed:

The three consumers computed the enumeration scope and threw it away, so
TRUNCATED/UNSCOPED/UNREADABLE collapsed into the same output as COMPLETE --
reproducing this epic's own output-identical-failure defect one layer up.
progress, stats and phases list now emit phase_scope (null on the phases list
--phase lookup path, which performs no enumeration).

Two routings were wrong and the suite proved it:

roadmap-parser's two milestone phase-count scans are reverted to the 999-only
literal. isSentinelPhaseId is BROADER than what it replaced: its legacy branch
runs /^0*(\d+)/ over "00.1", which backtracks to capture 0, so it read #2554's
decimal phase ids as sentinel milestone 0 and stopped counting them.

state.cts phaseInventoryProvider is reverted to the physical disk scan.
`state rebuild` is a RECONCILIATION pass -- scoping it made it throw on healthy
trees whose fixture resolves no window, swallowed the raw readdirSync fault
message #3057 B1 requires verbatim, and stopped it dropping orphan STATE.md
rows, which is the job.

Both are now function-scoped guard exemptions with written reasons, not
silent reverts. This is the consolidation trap named in the epic: a canonical
rule can cover MORE than the copy it replaces, and only real inputs show it.

Adds phases list coverage, a scope-branch test, and a drift-guard unit suite;
backports comment-awareness to the milestone-window and plan-count guards so
all three siblings share one false-positive profile; names #3161 alongside
#3167 in Amendment 3's subsumption record.

* fix(#3185): correct isSentinelPhaseId's decimal-zero misclassification

An isolated security review caught this branch committing the epic's own sin:
the over-broad predicate was worked around at ONE call site and left live at
the destructive ones.

isSentinelPhaseId's legacy branch ran /^0*(\d+)/, which backtracks so any id
whose leading digit run is all zeros before a non-digit captures 0 -- "0.1",
"00.1" and "0.2554" all read as sentinel milestone 0. Two pinned contracts
disagree with that: #2554 requires "00.1" to be counted as a real phase, and
the 999 icebox is a whole reserved milestone so "999.1" must stay sentinel.

The rule is asymmetric and now says so explicitly: 999 is sentinel with or
without a decimal part; 0 is sentinel only when bare. A decimal phase under
either is a real phase for 0 and reserved for 999, because 999 reserves a
MILESTONE while 0 reserves a PHASE.

Fixing the owner lets the earlier workaround go: getMilestonePhaseFilter's two
scans route through isSentinelPhaseId again and the guard exemption that
existed only to accommodate the defect is deleted. The state.cts cmdStateRebuild
exemption stays -- that one is a genuine reconciliation-wants-the-physical-set
case.

Also corrects tests/adr-612-bracket-grammar.test.cjs, which asserted
isSentinelPhaseId('0.1') === true and so had encoded the defect as expected
behavior.

* fix(#3185): keep isSentinelPhaseId's semantics — 0.x is layered, not wrong

Reverts the previous commit. The remote suite failed six tests proving it
wrong, and the reason is the sharpest finding of this phase.

An isolated security review observed that isSentinelPhaseId reads 0.1 and 00.1
as sentinel milestone 0 and judged that a defect against #2554. Correcting the
canonical predicate broke #2949. Both contracts are pinned and both are right,
because they ask different questions:

  #2554  is this dir part of the current milestone's phase SET?  -> count 00.1
  #2949  must this phase COMPLETE before the milestone closes?   -> 0.x sentinel

No single global predicate answers both. isSentinelPhaseId keeps its semantics
(0.x IS a sentinel, #2949), and the milestone-window layer keeps a narrower
999-only rule (#2554) as a function-scoped guard exemption with a written
reason — not a second silent copy.

That corrects how Decision 1 reads: "one owner per derivation" governs who
computes an answer, not how many questions share it. An over-broad canonical
rule is as much a defect as a divergent copy and fails worse, because it looks
like consolidation. Recorded in Amendment 3 as the lesson for Phases 4 and 5.

Where a review's inference about intent conflicts with a pinned contract, the
pinned contract wins; the finding is adjudicated, not fixed.

The boundary tables in the enumeration suite are corrected to assert 0.x IS a
sentinel, with the layering explained.

* refactor(#3183): route live-plan counting through scanPhasePlans

scanPhasePlans becomes the sole owner of the live-plan derivation. Twenty-one
independent re-derivations across seven modules now route through it, and
scripts/lint-plan-count-drift.cjs reports zero, scanning the whole repo rather
than an allowlist (ADR-3180 Decision 4a).

The epic scoped this at three copies. A whole-repo guard found twenty-six sites
across nine files, so Phase 1 absorbs every live-plan re-derivation and Phase 3
narrows to window plus sentinel enumeration.

Two sites are exempt with a documented reason rather than a bare allowlist:
audit.cts scans one quick task's own directory for a single completion record,
and gsd2-import.cts reads a foreign GSD-2 tasks/ layout during a one-time
import. Neither is a phase directory.

scanPhasePlans gains allPlanFiles (pre-supersession) alongside planFiles so one
owner answers both questions: verify.cts's numbering-gap check wants every plan
on disk, its pairing check wants the live set. Both fields are additive.

Highest-severity fix: cmdPhasePlanIndex, which feeds execute-phase wave
scheduling, was scheduling status:superseded plans into waves and reporting zero
plans for the post-#3139 nested layout.

filterPlanFiles and filterSummaryFiles are deleted; getPhaseFileStats orphaned
them and only their own tests still called them.

New leaf module src/planning-scope.cts carries the frozen SCOPE discriminator,
with its six-gate ripple closed: gitignore, inventory manifest, INVENTORY.md and
the CONTEXT.md glossary.

Records the amended scope, scanPhasePlans's new allPlanFiles field,
findOrphanSummaries, the two documented exemptions, the re-derived Tier-2
table, and the describeNonCanonicalPlans trap for later phases.

The remote runner went red with 13 deterministic failures on both lanes,
and they were right: replacing verify.cts's canonicalPlanStem pairing with
summaryCandidates dropped a case the bespoke rule covered. A plan carrying a
descriptive slug after its id (68-01-scaffolding-PLAN.md) pairs with its
canonical-stem summary (68-01-SUMMARY.md), and summaryCandidates generated no
such candidate, so the plan read unsummarized.

The fix is to complete the one rule rather than restore a second:
summaryCandidates gains a canonical-id candidate, narrowed to fire only when an
id pair was actually extracted. countMatchedSummaries, findUnsummarizedPlans
and findOrphanSummaries all inherit it. The two-plans-one-summary collision
behaviour of the original rule is preserved deliberately and documented in
place.

Second defect, independently root-caused while verifying: routing the #2893
naming diagnostic through scanPhasePlans exposed it to the loose /PLAN/i
fallback, which is correct for counting and wrong for a naming check — a
non-canonically-named file was accepted as a valid plan and the diagnostic
went silent. cmdPhasesList, cmdFindPhase and cmdPhasePlanIndex now intersect
with a strict isCanonicalPlanFile predicate before reporting names.

Same class as the describeNonCanonicalPlans trap already recorded in ADR-3180:
a question about file naming wants the physical, strictly-matched set; only a
question about outstanding work wants the live set.

tests/repo-invariants.test.cjs asserts every bin/lib/*.cjs is linted xor
ignored per its ADR-457 migration state. The new planning-scope module closed
five of the six .cts ripple gates - gitignore, inventory manifest, INVENTORY.md
and the CONTEXT.md glossary - but not eslint, because that one is enforced by a
test rather than by lint:ci, so the local pipeline stayed green while it was
missing.

Generated from src/planning-scope.cts, so the .cjs is ignored and the .cts is
linted, matching every other migrated module.

* fix(#3183): replace the plan-count drift detector with a literal tokenizer

CodeQL reported 4 high-severity js/redos alerts on REGEX_LITERAL_MD_RE, the
backtracking regex that finds "a regex literal mentioning PLAN/SUMMARY and an
escaped \.md". Five review rounds found it had two defects, not one:

  - EXPONENTIAL, then CUBIC. Its "any char" atom `(?:\\.|[^/\r\n])` let a `\.`
    pair be consumed either as one escape or as two class characters, which is
    exponential backtracking: 27,464ms on `"/\.mdplan" + "\.".repeat(28) + "X"`.
    Excluding `\` from the class killed that but left a cubic path — 23ms at
    N=200, 172ms at N=400, 1362ms at N=800 on `"/" + "PLAN\.md".repeat(N)` with
    no closing `/`. This guard is the last stage of `npm run lint:ci`, which CI
    runs on fork pull requests, so a crafted src/*.cts could stall the job.
  - A DETECTION HOLE. A character class holding a bare, unescaped `/` — e.g.
    `/SUMMARY[^/]*\.md$/`, an ordinary path-excluding filter — terminated the
    literal at that `/`, so the scan never reached `\.md` and the guard missed
    it entirely. (Classes holding an ESCAPED `\/` were already matched; the
    tests cover those separately as parity, not as regressions.)

Both defects have one root cause: regex-literal grammar — `\x` escapes, and
`/` inside `[...]` not terminating — is not expressible in a backtracking
regex. So the detector is now a tokenizer, not a regex.

readRegexLiteralAt reads the literal at a given `/` in a single left-to-right
pass with no backtracking, treating escapes as two-character units and
suppressing the `/` terminator inside a character class. findRegexLiteralMdMatch
restarts it at every `/` on the line, preserving the old "find anywhere"
behaviour; MAX_REGEX_LITERAL_LEN (400) bounds each read — including the
trailing-flag scan — which keeps the whole-line cost linear.

Results: cubic shape flat at 0.06-0.39ms out to N=3200 (25KB), exponential
shape 0.01ms at 28 reps and 0.00ms at 64, and the bare-`/` class shapes are now
caught. Differential against the old regex over 28,474 lines (those matching
FILENAME_TEST_RE but not PLAN_SUMMARY_LITERAL_RE, across src/tests/scripts/
gsd-core/bin/eslint-rules, excluding 265 lines with >6 backslashes on which the
old regex hangs): 6 differences, all the tokenizer returning the fuller or
newly-correct literal, 0 old-only misses. The `\.md` token stays
case-insensitive, matching the `/i` the old regex carried.

Also closes three holes in the same new file:

  - walk() tested entry.isFile(), false for a symlink, so a symlinked
    src/*.cts was silently unscanned — an evasion of a guard whose stated
    principle (ADR-3180 Decision 4a) is whole-repo discovery with no allowlist.
    It now resolves symlinks, but confined: file links must resolve inside the
    repo root, directory links inside the scanned dir itself. Every sibling
    drift guard in scripts/ uses the Dirent classification and never follows
    links, so following them unconfined would have made this the only linter
    able to read outside the tree — on fork PRs an arbitrary out-of-repo read
    whose matched fragments reach a public CI log. The narrower directory rule
    additionally stops `src/up -> ..` from sweeping the whole repo, and the
    skip list is now checked against resolved paths so `src/g -> ../.git`
    cannot reach .git/** or node_modules/**. Real paths are de-duplicated and
    files reported canonically, so a symlink alias cannot shift which
    FUNCTION_SCOPED_EXEMPTIONS key applies.
  - Both the reported fragment and the reported FILE PATH are attacker-
    controlled source text written straight to a CI log, and git permits
    control bytes in a filename. Both are now escaped — C0/C1/DEL plus the
    bidi and zero-width controls — so a crafted literal or filename cannot
    recolour the log, overwrite a line with CR, or fabricate a line that looks
    like this guard's own success output.

Regression coverage in tests/plan-count-single-owner.test.cjs: a child-process
probe over both pathological shapes (catastrophic backtracking is synchronous
and would freeze the suite rather than fail one test), the bare-`/` class
shapes verified to fail against the parent-commit blob, root-confinement tests
covering the outside-file, outside-directory, cycle, broken-link and duplicate
cases, direct isInsideRoot coverage including the sibling-prefix case that a
bare startsWith would let through, sanitizeForReport coverage, and
limit-1/limit/limit+1 coverage of MAX_REGEX_LITERAL_LEN derived from the
exported constant. The earlier structural assertion was dropped — it checked
for the substring `[^/`, which respelling the class as `[^\r\n/]` defeats
while staying exponential.

0e6fa2e2cf enhance(#3118): close the dead injectables and the shell projection follow-on — Wave 4 (#3124)
* test(#3118): failing-first coverage for the dead injectables and the shell projection

Adds the counter-tests Wave 4 closes against, before any fix:

- antigravityWatermark had zero test references. The four existing tests
  that look like watermark coverage hand the fallback a literal mark and
  never call the producer, so nothing pinned whether a real run's mark is
  correct. Covers all six branches plus the non-object cache classes.
- Pins the fail-open: a transcript read that throws reports lines:0,
  indistinguishable from a genuinely empty transcript, and the consumer
  then replays a previous run's review as this run's.
- Pins the export-line escaping across the repair, persist and win32 bash
  lanes, including the parity assertion that they must not diverge.
- sliceCurrentPositionSection: empty-vs-absent, fenced heading, second
  occurrence, H3, CRLF.
- Proves deps.progressProvider is inert by supplying a throwing stub to
  all ten transition intents.

Verification through the remote runner only.

* fix(#3118): distinguish an unreadable transcript from an empty one

antigravityWatermark's final read can throw on a transcript that
indisputably exists. It returned lines:0, which is the same value a
genuinely empty transcript produces, so the caller could not tell the
two apart.

antigravityTranscriptFallback derives its skip from that count. A mark
of {convId:'c1', lines:0} for a conversation that pre-dates the run
makes it skip nothing and return the last PLANNER_RESPONSE in a
transcript written before this run started — a previous review
presented as this one's, which is exactly what the function's own
'never stale' docstring promises cannot happen.

The unreadable case now sets unreadable:true and the fallback declines
for a same-conv-id unreadable mark. An absent or empty transcript is
untouched: those genuinely have zero prior lines.

* fix(#3118): escape the export line for the file it lands in, not the echo

Three lanes emit export PATH="<dir>:$PATH". repair escaped it with
escapePosixDoubleQuoted; persist and the win32 Git Bash lane escaped it
with escapeSingleQuotedShellLiteral instead.

The single-quoting is correct for the echo, so nothing runs when the
user pastes the command. But the bytes appended to ~/.bashrc are the
export line itself, and inside double quotes in an rc file a $(...) or
a backtick in the directory name is command substitution that runs on
every new shell. Those characters are legal in a path on both POSIX and
Windows, so the path was reachable.

projectPathExportLine is now the single source of that line and escapes
for its final rc-file context; each lane still applies its own transport
escaping on top. fish keeps the single-quote escaper — its value really
does stay single-quoted.

The cmd.exe lane interpolated into a cmd double-quoted string with no
cmd-level escaping, so a quote closed the region and &cmd& ran. A quote
is reserved on Windows and cannot appear in a real path, so there is no
correct command to suggest: the win32 lanes now fail closed for one.

Metacharacter-free paths render byte-identically on every lane.

* fix(#3118): drop a stray carriage return and a deps field nobody reads

locateCurrentPosition subtracted a fixed one byte to exclude the newline
before the next heading, which assumes LF. On a CRLF document the slice
kept an unpaired trailing carriage return. It now walks back over the
newline and over a preceding carriage return if there is one.

StateTransitionDeps also required a progressProvider that 33 sites
supplied and no site ever called. A required field nothing reads widens
the module's interface without changing its implementation, which is the
shape epic #3051 cites as its reason for refusing blanket injection.
Removed along with the ProgressRecord alias that existed only as its
return type; state-document.cts's unrelated interface of the same name
is untouched.

* fix(#3118): stop an empty span duplicating bytes, and name the empty results

Three findings from the isolated review pass.

locateCurrentPosition could return end < start when the section was
empty and the next heading followed with no blank line between. Every
mutator splices with slice(0,start) + body + slice(end), so an inverted
span duplicated the region between them — a blank line silently
inserted into STATE.md on every transition, two bytes on CRLF. The span
is now clamped, and an empty section is a zero-length span, which is
what it always meant.

The win32 fail-closed path left the installer printing 'Add it with one
of:' with nothing under it. An empty shellActions folded two different
facts together, so projectPathActionProjection now carries a frozen
PATH_ACTION_REASON and the installer branches on it. Two empty results
with different causes staying distinguishable is the subject of the
epic this belongs to.

fish_add_path parses a leading dash as an option, so a directory named
-v printed 'No paths to add' instead of being added. Verified against
fish 4.8.1: the end-of-options separator fixes it.

Replaces the console-prose test the second fix first arrived with — a
regex over captured stdout is what CONTRIBUTING prohibits, and the
typed reason is the surface it asks for instead.

* fix(#3118): escape TOML control characters, and stop a test name overstating

Five findings from the two review axes.

escapeTomlDoubleQuotedString escaped only backslash and quote. TOML
basic strings also require U+0000-U+0008, U+000A-U+001F and U+007F to be
escaped, so a value carrying a raw newline or NUL wrote a config.toml no
parser accepts — rejecting the whole file, not just that value. Four of
its call sites write real config. Tab stays raw; the grammar exempts it.

The byte-identity test claimed every lane was unchanged for an ordinary
path, which is false: fish now takes the end-of-options separator on
every path, not only hostile ones. Renamed, and the one intended delta
now has its own named test instead of hiding inside a claim that read
as broader than it was.

Also: exact-equality assertions in place of substring checks that could
pass on a subtly wrong escape, newline and null-byte cases for all five
quoting primitives, and a temp dir registered with t.after so it is
removed when an assertion fails.

* docs(#3118): add the changeset fragments

* fix(#3118): degrade instead of throwing on a null conversation cache

A cache file whose whole content is the literal null — what a truncated
or zeroed write leaves behind — made both antigravityWatermark and
antigravityTranscriptFallback throw. JSON.parse('null') succeeds, so the
try/catch wrapping the parse never fired, and resolveConvId then called
hasOwnProperty on null.

Both functions advertise the opposite; the existing test next to them is
named 'a missing cache or transcript degrades to empty, never throws'.
Parsing successfully is not the same fact as the payload being usable,
and a guard that only wraps the parse cannot tell them apart.

resolveConvId is now total for any non-object input, so one guard covers
both callers. Caught by the null case in this wave's own cache matrix.

* test(#3118): correct a stale fish expectation and a parity comparison

The pre-existing 'POSIX persist mode escapes single quotes' test pinned
fish_add_path without the end-of-options separator this wave adds, so it
asserted behavior that is no longer correct. A repo-wide scan found one
such hardcoded expectation; every other site derives its expectation
from the projection.

The new parity test compared the token from a POSIX path against the
win32 lane, which posix-normalizes its input first — two different
inputs, so the tokens differed for a reason that had nothing to do with
the parity it claims to check. It now derives the win32 expectation from
the same input the lane receives.

* docs(#3118): reword a comment the injection scanner reads as an instruction

The scanner pattern act\s+as\s+(?:a|an|the)\s+ carries no word
boundary, so 'the same fact as the payload' matched on the tail of
'fact'. Reworded per the documented remedy for this collision.

The missing boundary is a scanner defect rather than a prose problem —
any contributor writing 'fact as the' trips it — but the pattern is
gate plumbing, which the sibling epic owns, so it is surfaced rather
than changed here.

Three defects in the negation suppression added by #3127, all in one
block, none of which had a test.

The pair scan was verbs.some(nouns.some(...)) with a slice and a split
per pair, so it grew cubically with clause length: 1.1ms before that PR
and 8462ms after, on 800 verb+noun pairs in one clause. api-coverage's
property test generates documents large enough to reach the runner's
600s file cap, which is why it hangs as 'fail 0, cancelled 1' rather
than failing an assertion. Every (verb, noun) window is a subset of the
single widest one, so one scan of that window answers the same question
in a linear pass. Verified equivalent against the old predicate over
20,000 generated clauses.

Both checks also subtracted clause.start from offsets that collectTerm-
Matches already returns clause-local. The first clause on a line has
start 0 so it worked there and nowhere else: later clauses went
negative, and slice reads a negative index from the end, so suppression
silently examined unrelated text.

The comment claimed 'without any API integration' was suppressed. It is
not — the qualifier sits outside the two-word lookback and the noun
precedes the verb. Widening the window would trade a false positive
that costs one declaration line for a false negative that slips a real
integration past a blocking gate, so the behavior stands and the
comment now says so. Pinned by a test.

The qualifier sets were also rebuilt for every line of every document.

writeManifest documents itself as a fail-closed duplicate guard: if any
existing manifest shares plan_id with a different, non-terminal job_id it must
refuse, because dispatching again would duplicate the external job.

It could not honour that. The scan reads every sibling manifest looking for the
duplicate, and an unreadable or unparseable sibling was `continue`d past. If
the corrupt file was the one holding the live duplicate, the scan found nothing
and a duplicate external job dispatched.

The asymmetry is what gives it away: a malformed TARGET refused with
malformed_existing because clobbering is unacceptable, while a malformed
SIBLING was skipped — yet siblings are the only thing the duplicate check
reads.

Adds a scan_incomplete verdict that refuses and names the offending file, so an
operator can quarantine or repair it. Fail-closed alone would let one stale
corrupt manifest wedge every dispatch for that planning dir permanently; naming
the file is what makes refusing survivable. malformed_existing is untouched, so
the target/sibling distinction stays visible. The docstring is updated — it
previously stated a rule the function did not keep.

memFs() gains an optional failReads map so these branches are reachable at all;
they had zero coverage because the fake could not express a per-file read
fault. The signature is additive and every existing caller is unchanged.

The regression is proved by a pair, not a single test. A control writes a
readable sibling holding a genuine non-terminal duplicate and asserts
duplicate_plan_id, establishing the scenario is real; the regression then makes
that same path unreadable and asserts scan_incomplete. A first draft of this
test used a corrupt-JSON fixture containing no plan_id at all while its comment
claimed otherwise — it duplicated the unparseable-sibling case and proved
nothing, which is the defect class this phase exists to remove.

Wave 1 of the negative-space backfill: the branches where a guard that could
not verify something reported the same value it reports when everything is
fine. That indistinguishability is the defect; every fix here makes the two
states tellable apart, and every test proves it with a pair — one for the
failure, one for the benign case. A single test cannot establish that two
states are distinguishable, which is the whole property being fixed.

state.cts phaseInventoryProvider returned null for both a real disk-scan
failure and a genuinely empty phases dir, so `state rebuild` could report
success while phase-table reconciliation never ran. It now returns a
discriminated result and the CLI surfaces phase_inventory_scan_failed plus a
reason. The reason field turned out never to have been wired into the emitted
JSON at all — it existed only as an internal variable — so a test could only
assert on the operator-facing note. It is a real field now.

state.cts treated an unreadable lock body the same as an empty one, applying
the 1-second stealable floor. A lock we cannot read is not a lock we know is
stale; an unreadable body is now held to the deadman ceiling like a live
holder.

verification.cts findStaleVerificationSummary returned null on any fs, scan or
clock failure — meaning "not stale". It now returns a discriminated
StaleCheckResult and the caller records that the check was indeterminate.

git-base-branch resolveBaseBranch returned 'main' both when no candidate branch
existed and when every git tier timed out. A diagnostics variant now reports
whether the answer was verified, and the CLI writes an unverified-fallback note
to stderr. The stdout contract five workflows parse is untouched.

worktree-safety snapshotWorktreeInventory left exists:true when statSync threw,
so a guard that could not check reported the worktree present; exists is now
tri-state and a stat failure surfaces as an 'unverified' finding.
planWorktreePrune reported 'no_worktrees' for a parse failure, which is not the
same as an empty list — and it drives a prune. It now reports 'parse_failed'.

Fixing the inventory change exposed a second fail-open in verify.cts: the
validate-health consumer silently dropped findings whose kind it did not
recognise, so the new kind would have vanished. That is closed too — worth
noting that the survey enumerated producers of degraded verdicts, not consumers
that discard them.

worktree-base-ref and state-transition gain the distinguishing signal without
changing what they do: headAbsenceVerified, and a phase-inventory scan meta.
Whether those guards should ACT differently is a product question this change
does not answer, and both are flagged rather than quietly settled.

rescueSummaryArtifacts is left alone: rescuing on an uncertain cat-file is
deliberate per #2556. It now has tests proving it, and a recorded negative
finding — git cat-file -e returns 128 for both "absent from HEAD" and a fatal
error, so "uncertain" and "certain-and-fine" are not separable at the git
level.

Ten assertions in the rebuild CLI suite matched substrings of produced output —
STATE.md body fields, a markdown table row, an audit-log heading, and JSON keys
read as text. CONTRIBUTING prohibits that: if the code under test produces
text, the test asserts on its structured surface instead.

No production surface had to be built. Every one already existed and was
already compiled into bin/lib: stateExtractField for body fields,
parseMarkdownTable for the phase table, collectSection for the audit-log
section, and result.data.log — already a typed RebuildLogEntry[]. The tests
were matching rendered text sitting next to the structured data.

One of those assertions was passing for the wrong reason. `stdout.includes
('rebuilt')` matched the JSON KEY name, not a value: the dry-run path emits
`mutated` and the real path emits `rebuilt`, so it would have passed whether
the value was true or false. It now asserts the value.

external-job's refusal already had to name the offending file — that naming is
why the fail-closed variant is survivable rather than a permanent wedge — but
the tests proved it by substring of a prose message. The failure result now
carries offendingPath as its own field and the tests assert it by value. The
human message is unchanged; operators read it.

Array membership is left alone. `phaseIds.includes('99')` and
`result.updated.includes('Completed Phases')` are membership checks on real
arrays, not text matching, and converting them would weaken nothing and clarify
nothing.

called acquireStateLock. It carried an allow-test-rule: architectural-invariant
exemption to permit that. A source grep proves a literal is present in a file,
not that the behaviour works — it is weaker than a liveness test, which at
least runs the code, and it was the only coverage the fatal-errno path had.

Replaced with tests that inject the errno through fs and assert what actually
happens: a fatal EACCES propagates out of acquireStateLock with zero backoff
sleeps, while EAGAIN/EINTR/EINVAL/EIO/ENOENT/ESTALE/EPERM/EBUSY retry once and
succeed. The exemption is removed and its allowlist entry with it.

One old assertion is deliberately not carried over: it checked the retryable
errnos were expressed as a Set rather than an inline literal. That is a shape
check with no runtime signature; the behavioural tests fail if the code reverts
to the old inline check, which is the regression it was really guarding.

The #3057 lock-body tests move into that same file rather than a new one, which
is what lint-test-file-count asks for and puts every acquireStateLock test in
one place.

A signal nobody consumes leaves the fail-open exactly as silent as before: the
staleness check could fail and the operator saw precisely what they would see
if the answer were genuinely "not stale". That is the defect this issue exists
to remove, so it is not defensible as scaffolding when its two siblings in the
same change already wire through.

All five callers now surface it, each through the channel it already had rather
than a mechanism imposed uniformly: phase complete adds it to its existing
warnings array and, on the blocked path, as an additive note on the error text;
init and roadmap carry it as a field on output they already emit; the UAT
report carries it without ever gating passed/blockers; workstream inventory
takes an injectable writeDiagnostic mirroring the git base-branch idiom,
because its return shape had nowhere to hang a per-phase field without
rippling the builder's types.

The routing decision is unchanged everywhere. What changes is only that a
caller and an operator can now tell a failed check from a completed one.

That diagnostic carries structured meta rather than being asserted by regex —
the default still writes only the human message to stderr, but tests assert
phaseDir and reason by value. Two earlier assertions in this branch were
converted the same way; this was the last raw-text assertion left.

Also records a scope correction: the completePhaseCore guards now compare
stateReplaceField's result to the body instead of testing truthiness, so a
field whose substitution produced identical text no longer reports as updated.
That is a real behaviour fix, not the signal-only change this file was
described as carrying, and its tests cover both the changed and unchanged
cases.

The remote matrix surfaced three failures unrelated to this branch's changes.
All were bad tests, and a re-run would have hidden every one of them.

The reviewer-flags parse block bounded bash -> node -> a full gsd-tools cold
start at 5 seconds. On a bench running thirty thousand tests in parallel that
is not a hang, it is a busy machine. Raised to 30s, matching the convention
sibling suites already use for script invocations, with a comment saying what
the budget covers so nobody tightens it back. Two further copies of the same
5-second spawn in the same file had the identical defect and are raised too —
they were not in the failure report, but they will be next time.

The fragment-propagation test bounded npm run regen:derived — a full build plus
eight generators, the heaviest subprocess in the suite — at five minutes, and
node22 was killed near the end. The captured output proves it: every generator
had written its files and gen:install-tree had emitted all fifteen runtimes
before the kill. Raised to fifteen minutes.

That failure read as `null !== 0`, which says nothing. status null means killed,
not a non-zero exit, and the two want different responses: one is a timeout to
size correctly, the other is a real build break. The assertion now distinguishes
them and names the signal.

Neither test's assertions were weakened and no retry was added. A retry here
would suppress exactly the signal the timeout exists to produce.

The phase suite reported zero test results on both lanes while running for five
and a half minutes and exiting 1. No assertion text, no stderr, four events for
the whole file: enqueue, start, dequeue, complete. That shape is not a failing
assertion — it is the runner being unable to read the child at all, because it
parses its event stream from the child's stdout.

The cause was the capture helper reassigning fs.writeSync directly. Proven
rather than assumed: a standalone probe patched fs.writeSync and called
process.stdout.write, and the interception fired only when fd 1 resolved to a
FILE, not when it was a pipe. The remote runner captures the event stream to a
file, so a helper that was invisible against a pipe swallowed the reporter's own
output on the bench. That is also why the two sibling suites wired the same way
in this change pass cleanly — they use the mock tracker, the seam io.test.cjs
established for this exact function.

The helper now uses t.mock.method with an explicit restore after each call, so
teardown belongs to node:test rather than a second hand-rolled implementation,
and the interception cannot outlive the one synchronous call it wraps even if
that call throws. Ten call sites thread the test context through; three test
callbacks gained the parameter they lacked.

The three B3 tests are untouched — same assertions, same fault injection. Only
how the context reaches the helper changed.

Two attempts to make in-process fd-1 interception safe both failed on the
bench. The suite reported zero test results on either lane while exiting 1 —
four events for the whole file — because the runner parses its event stream
from the child's stdout, and process.stdout.write routes through fs.writeSync
whenever fd 1 resolves to a file, which is how the runner captures. Patching
that seam anywhere in a file can therefore destroy the file's own reporting,
and tightening the window only moved the runtime from 326s to 125s without
recovering a single event.

So the interception is gone rather than tuned. The helper now spawns gsd-tools
as a real subprocess and reads stdout the way the OS already gives it to us,
which is what the rest of the suite does. It asserts the command succeeded
before parsing, so a genuine failure can no longer present as a JSON parse
error.

The two fault-injecting tests could not survive that move as written: a
subprocess cannot see a mock installed in the parent. Instead of reinstating
the interception they now produce the fault on disk — the summary artifact is
created as a dangling symlink, so the staleness check's real statSync throws
inside the child. That is a more honest fixture than a mock in any case, since
it is a condition a user's tree can actually be in. Skipped on Windows, matching
the existing symlink precedent in the write-guard suite.

Three further call sites turned out to depend on parent-process writeFileSync
mocks the subprocess could not see. Those call the CJS function directly, which
is what they always wanted — they never needed stdout at all.

Standards review found four things this branch introduced, all of them
inconsistencies with itself rather than with the repo.

One upstream bit reached its consumers under three names —
verification_stale_check_indeterminate in two modules, the same value with
"stale" dropped in a third, and stderr only in the fourth. Standardised on the
long name wherever it is a field. The workstream inventory keeps its stderr
channel, since its return shape has nowhere to hang a per-phase field without
rippling the builder's types, but it now says the same word for the same thing.

worktree-safety encoded one three-way distinction two ways in a single file: a
named union for a finding's kind, and boolean|null for an inventory entry's
existence. The second is now a named union too.

Two assertions matched human prose because the blocked and non-blocked
completion paths carried no typed field for the signal. Both now assert typed
values. The first round of this fix added the field but left the regex beside
it, which is the banned pattern sitting next to its own replacement; the second
removed it and added an assertion on the reason enum so nothing was lost.

The remaining two were reasoned away before being fixed, and both reasons were
bad. "No typed surface exists" is the condition CONTRIBUTING says to fix by
adding one — it took three lines. "The file already does this dozens of times"
is not licence to add instance number thirty-one; a convention that violates a
documented rule is debt, not precedent.

Vocabulary differing across DIFFERENT modules is left alone: CONTEXT.md rejects
a single shared result envelope, so per-module shapes are precedented, and a
baseline smell does not outrank a documented standard.

A census of every line this branch adds to a test file now finds no regex or
substring assertion on produced prose: 87 strictEqual, 25 ok (all non-empty or
shape guards), 12 equal, 3 throws (all typed err.code predicates), 3
deepStrictEqual, 2 notStrictEqual.

but rejected the write), restore beforeCheckbox. The two surfaces can no longer
silently diverge. Forward-status rows (Pending/In Progress/Gaps Found) still flip
+ advance; absent rows still flip (nothing to disagree with).

Failing-first regression for #2949. cmdPhaseComplete's stage-3 lowest-outstanding
loop has no sentinel filter, so completing the last real phase with an unchecked
0.x backlog row present selects the sentinel as next_phase, corrupting STATE.md.
Row 1 asserts the 0.x sentinel is not selected; Row 2 guards the #2028 real-lower-
phase out-of-order behavior; Rows 3-4 cover STATE desync and the checked-sentinel
boundary.

* fix(#2949): exclude sentinel phase ids from stage-3 next-phase candidacy

cmdPhaseComplete's stage-3 lowest-outstanding-override loop (#2028) had no
sentinel filter, so completing the last real phase with an unchecked 0.x backlog
row present selected the sentinel as next_phase — comparePhaseNum("0.1","12")
=== -12 sorts it below every real phase — corrupting STATE.md and desyncing
current_phase from current_phase_name.

Add !isSentinelPhaseId(cbm[2]) to the stage-3 condition, reusing the existing
zero-caller predicate (SENTINEL_RANGES = [0, 999]) so both sentinel ranges are
excluded. A real lower-numbered outstanding phase is not a sentinel and is still
selected, preserving #2028's out-of-order-completion behavior.

Stage-3 only: PR #2815 (in-flight, #2786) covers stages 1-2; the two PRs touch
disjoint code.

* fix(#2949): compare next_phase numerically in Row 2 (handles padded/unpadded)

Row 2's assertion /09/.test(next_phase) failed because the CLI returns the
unpadded "9", not "09". Compare numerically (parseInt === 9) so the assertion
holds for either form.

* fix(#2949): mark Phase 11 complete in Row 1 so only the 0.x sentinel is unchecked

Row 1's fixture left Phase 11 unchecked, so completing Phase 12 correctly
selected Phase 11 as the real lower outstanding phase (is_last_phase=false) —
the assertion is_last_phase===true was wrong for that fixture, not the code.
Mark Phase 11 [x] so the ONLY unchecked lower row is the 0.x sentinel, which is
the actual #2949 scenario. Confirmed locally: with Phase 11 checked + the fix,
completing 12 yields is_last_phase=true, next_phase=null (sentinel excluded).

ef823ca9d9 fix(#2830): propagate a halted plan to its transitive dependents (#3038)
* test(#2830): add failing regression tests for halted-plan dependent blocking

Add tests/fix-2830-halted-plan-dependents.test.cjs covering direct,
transitive (2 and 3 hop), and diamond dependents of a halted plan across
both independent "which plans are incomplete" readers (phase-plan-index's
cmdPhasePlanIndex and findPhaseInternal/searchPhaseInDir), the negative
case (an unrelated decoupled plan stays runnable), and a parity check that
the two readers agree. Uses only modules that already exist at this
commit (gsd-tools.cjs via subprocess, the pre-existing phase-locator.cjs)
so the test file loads and runs cleanly on a fresh clone of this exact
commit. These fail against current behavior: neither reader has any
concept of a halted plan or a blocked_by/runnable view yet.

* fix(#2830): a halted plan no longer leaves its dependents on the runnable work list

A plan that reaches a designed stop still writes a SUMMARY, so both
"which plans are incomplete" readers saw it as an ordinary completion and
reported its dependents as ordinary runnable work — never checking
whether an upstream plan had halted rather than finished.

- New `status: halted` frontmatter value, documented in all four SUMMARY
  templates alongside the existing `status: complete`.
- New shared src/plan-dependency-graph.cts: a single computeHaltPropagation
  pass that both phase.cts's cmdPhasePlanIndex (wave-grouping) and
  phase-locator.cts's searchPhaseInDir (the phase-location primitive, ~50
  dependent symbols across 5 command routers) now call, so the
  two-implementation divergence that caused this bug cannot recur. It
  accepts an optional precomputedOrder so cmdPhasePlanIndex — which already
  runs Kahn's algorithm in computeDependencyLevels for wave assignment —
  passes that order straight through instead of a second traversal;
  searchPhaseInDir (no prior traversal) lets the module derive its own.
  The two small duplicated predicates each reader would otherwise carry
  (is this status "halted"?, which summary file matches which plan id?)
  are centralized in the same module as isHaltedStatus/buildSummaryFileIndex.
- Additive fields only: `halted`/`blocked_by`/`runnable` on
  cmdPhasePlanIndex's plans[] and top level, `halted_plans`/`blocked_by`/
  `runnable_plans` on searchPhaseInDir's result. The pre-existing
  `incomplete`/`incomplete_plans` fields are unchanged in meaning and
  membership.
- execute-phase.md's discover_and_group_plans step now also skips any
  plan whose `blocked_by` is non-empty, reporting it by name with its
  blocking chain, in addition to (not instead of) the existing
  has_summary skip rule.

Extends tests/fix-2830-halted-plan-dependents.test.cjs (introduced in the
prior commit) with direct unit coverage of computeHaltPropagation
(including the precomputedOrder call shape) and a fast-check property
test — both only possible once this commit's new module exists.

Closes #2830

* fix(#2830): surface the halt-aware view from init execute-phase

The adopted work made phase-locator compute halted_plans / blocked_by /
runnable_plans, but cmdInitExecutePhase builds its output by explicitly
enumerating fields, so all three were computed and then silently dropped at
the exact consumer the issue names as regressed.

Forwards them additively -- incomplete_plans and incomplete_count keep their
name, type and semantics byte-for-byte -- and adds the same three empty
defaults to the roadmap-only fallback so the shape is consistent in both
branches. Covered by a new test that drives the real CLI end to end rather
than the locator function, since the locator already worked.

- BLOCKER (isolated adversarial). Cycle participants never reach indegree 0 in
  the Kahn pass, so they were excluded from the topological order, never visited
  by the forward pass, and vanished from blocked_by entirely -- i.e. reported as
  runnable. The wave-grouping reader hard-fails on a cycle so it never hit this,
  but the phase-location reader does not, so init execute-phase offered a plan
  depending directly on a halted plan. Reproduced, then fixed in the shared
  engine so every consumer is safe regardless of pre-checks: a node absent from
  the order is now blocked with a deterministic, non-empty named cause. A plan
  silently missing from both blocked_by and runnable is the exact disappearance
  this issue exists to prevent.

- MAJOR (isolated adversarial). All four summary templates showed the field as
  an inline comment on the value line. Frontmatter parsing does not strip
  trailing comments, so an executor copying the templates' own presentation
  wrote a halt that parsed as a non-halted string, silently reproducing the
  original bug. Guidance moved off the value line, and the halt predicate now
  tolerates an unquoted trailing comment.

- HARD standards violation. A test regex-matched child-process stderr prose for
  /cycle/i, which CONTRIBUTING bans. Replaced with the structured failure signal
  plus a differential assertion (same fixture without the cycle edge must
  succeed), so it stays cycle-specific without matching prose.

Also folds the duplicated read-summary-and-check-halted wrapper out of both
readers into the shared module -- centralizing only the predicate left the exact
two-copies-that-drift pattern the module exists to prevent -- and commits the
artifact-types documentation for the new status value.

The remote runner did not fail -- it hung. Two containers sat in this file for
31+ minutes, and an earlier attempt ran 9 hours before I killed it. The runner
passes --test-timeout=0, so nothing ever reaps it: this would have hung CI
indefinitely, not reported a failure.

Root cause: the DAG generator built edges by rejection --

  from: fc.integer({ min: 0, max: n - 1 })
  to:   fc.integer({ min: 0, max: n - 1 })
  .filter(({ from, to }) => from < to)

With n === 1 both integers are forced to 0, so the predicate is unsatisfiable
and fast-check retries value generation forever. n is drawn from 1..12 and
fast-check biases toward boundary values, so n === 1 is reached almost at once.

This also explains why the failing-first run completed normally while the fixed
run hung: before the fix the graph module did not exist, so the property test
threw on import and never reached generation. It only starts hanging once the
code under test works.

Generates the DAG by construction instead -- `to` is drawn strictly above
`from`, with the degenerate single-node case short-circuited to an empty edge
list -- so no rejection sampling is involved. Switches the import to the shared
fast-check setup so the seed and run count are pinned per CONTRIBUTING, and adds
a bounded regression guard that samples the arbitrary directly, so a future
reintroduction fails loudly instead of hanging.

Verified: the file now completes in 2 seconds, 29 tests started and 29 finished,
zero failures, against an indefinite hang before.

Full-suite run surfaced two things the focused harnesses could not.

1. Regression of a pinned pre-existing contract (#3785). A refactor routed the
   EMITTED depends_on field through the new dependency resolver, which also
   consults the canonical-prefix map. The original consulted the plan map only,
   so a short canonical prefix passed through verbatim -- '24-01' stayed
   '24-01' rather than becoming '24-01-auth-hardening'. The emitted field is a
   DISPLAY mapping, not the DAG resolution, and #3785 pins that. Reverted with
   a comment recording why it must not use the resolver; full resolution is
   still used for the wave DAG and halt propagation, which is what needs it.

2. The workflow file grew 518 bytes without an acknowledgment, from the
   halt-aware skip rule and the widened parse contract. Acknowledged.

Note on where the acknowledgment landed: the guidance is to add a NEW fragment,
but execute-phase.md is already named by an existing fragment and the linter
hard-fails when two ack sources name the same path. Appending to the owning
fragment, following its own established multi-PR pattern, was the only
lint-clean option.

   boolean: true when content was written, false on no-op).

2. progress.* frontmatter stayed stale when the body lacked 'Total Phases:'
   or 'of N' — readModifyWriteStateMd's no-op guard (#948) skipped
   syncStateFrontmatter when the body transform was unchanged. Now the
   transform forces a body diff when a phase was actually removed, so the
   guard passes and syncStateFrontmatter rebuilds progress.* from the
   post-deletion disk/ROADMAP state.

Two MAJOR findings from isolated adversarial review:
1. Forced-diff injected a spurious 'Total Phases:' line even when no directory
   was removed (targetDir === null). Now gated on targetDir !== null.
2. Test #2 asserted 'not 3' instead of '2' — would pass for any wrong count.
   Now asserts exact value. Test #1 strengthened to assert body Total Phases

reason, plus a 3-case regression block in phase.test.cjs. The gate itself is
NOT yet wired into cmdPhaseComplete (reverted for the RED run), so the
'blocks completion' and 'superseded does not block' cases must FAIL (the
pre-fix code completes silently).

* fix(#2648): phase.complete refuses when non-retired plans lack summaries

cmdPhaseComplete gated only on a single *-VERIFICATION.md status, so a phase
could close complete while an arbitrary number of its plans had no completion
record (confirmed incident: 6/30 plans unexecuted incl. the phase's entire
final UI scope, every signal green). Add a fail-closed plan-coverage gate that
refuses completion when any plan lacks a matching *-SUMMARY.md, naming the
missing plans, UNLESS the plan is retired via machine-readable status:
superseded frontmatter (#2349) — closing the Goodhart hole (delete a SUMMARY to
raise the %) without regressing the lock/recovery pattern.

Uses scanPhasePlans (superseded-AWARE) + new findUnsummarizedPlans helper so the
gate, the count, and the named list can never disagree. Evaluated before the
verification-gate transaction so a refusal fails fast without mutating
ROADMAP/STATE. milestone.complete's parallel gap is explicitly out of scope
(separate seam, separate PR).

* fix(#2648): test fixtures — give #1752 phase plans summaries + STATE.md in coverage fixture

The plan-coverage gate (#2648) correctly blocks phase completion when a plan
lacks a SUMMARY. Two test fixtures needed updating to reflect the new contract:
- #1752 (total_phases-decrement cascade): its 8 phase dirs each had a PLAN.md
  with no SUMMARY. The test's concern is the total_phases cascade, not plan
  coverage, so add a matching SUMMARY to each to keep the phase fully-covered
  and isolate the #1752 behavior.
- the #2648 coverage-gate fixture: write STATE.md (createTempProject scaffolds
  .planning/phases but not STATE.md) so the 'ROADMAP/STATE unchanged on refusal'
  assertions have a file to read.

* fix(#2648): security — fail closed on unreadable plan dir + sanitize msg + surface superseded

Address the security-review blocker (B1) and hardening (M1/m2):
- B1 (blocker): the gate failed OPEN when scanPhasePlans could not read the
  phase dir (it swallows readdirSync errors → empty plan set → gate sees zero
  unsummarized plans → passes). A coverage gate that passes when it cannot read
  the plans re-opens the #2648 hole under any I/O failure. Now readdirSync the
  dir explicitly and fail closed (PHASE_PLAN_COVERAGE_INCOMPLETE) on a throw;
  a readable empty dir still passes (legitimately complete empty phase).
- m2: sanitize plan filenames (strip C0 controls / DEL) before interpolating
  into the error message — they come raw from readdirSync and could spoof the
  terminal in plain-error mode.
- M1: surface the count of plans excluded as status: superseded so a reviewer
  can audit which work was declared retired (the marker is a committable,
  review-time-trusted bypass; keep it visible).
- Add a 4th regression case: unreadable plan dir (ENOTDIR via a file, not chmod
  0o000 which root bypasses) must fail closed.

* test(#2648): drop unreachable B1 case — no root-safe unreadable-dir repro

The B1 fail-closed-on-unreadable-dir defense stays in src/phase.cts (cheap +
correct), but it cannot be unit-tested cross-platform: any condition that makes
the phase dir unreadable to the gate's readdirSync ALSO fails findPhaseInternal
upstream ('Phase N not found') before the gate runs, and chmod 0o000 is
forbidden (root bypasses it in root CI). Document the gap in the test file;
remove the case that asserted a reason the upstream error pre-empts.

* style(#2648): drop unnecessary type assertions flagged by lint:ci

scanPhasePlans returns typed string[] arrays, so the `as string[]` casts on
coverageScan.planFiles/summaryFiles were redundant (@typescript-eslint/
no-unnecessary-type-assertion). Compute supersededCount from typed lengths;
only the phaseInfo['plans'] cast remains (it is genuinely unknown).

Defect 1 (terminal state): requirements revert-phase (the gaps_found response)
left a row at 'Gaps Found' with no inverse — neither mark-complete's /^pending$/i
guard nor the phase-complete reconcile's /^(?:pending|in progress)$/i accepted it,
so a single failed verification stranded every requirement permanently and blocked
the milestone. Widen both guards to accept 'gaps found' so a genuinely-satisfied
stranded row reaches Complete again.

Defect 2 (false success): mark-complete ORed checkboxHit || tableHit for 'updated',
so on a Gaps Found row it flipped the checkbox but could not move the row, yet
reported updated:true. When a traceability table has a row for an ID, gate 'updated'
on the row moving (tableHit) — a checkbox-only flip on a table-bearing file no
longer lies. The #2140 table_unmatched path (no row for the ID) is preserved.

Primary: completePhase (adapter) and beginPhase (via readModifyWriteStateMd
options) pass the intent-held display name to syncStateFrontmatter as an
authoritative override, applied after every derive/preserve/carry-forward
step — so the lossy prose round-trip can never destroy a name the transition
just resolved. Names containing a parenthetical
(`Closer-ruling measurement (D1a)`) now land in frontmatter verbatim instead
of collapsing to the parenthetical (`D1a`).

Secondary (#1695 AC #3 residual): parsePhaseFromProse prefers the em-dash
name when it is a genuine name (not a status keyword, not a `Milestone:`
tail), else falls back to the parenthetical — satisfying both first-party
writer shapes (`N — Name (aside)` and `N (Name) — EXECUTING`). Still lossy
for paren-containing names, which is why the intent-first override is the
primary fix.

plannedPhase carries no name in its intent, so it is naturally out of scope.

Fixes #2736

StateTransitionResult.data is already `Record<string, unknown> | undefined`,

The artifact<->git check has existed since the beginning but was only ever
pointed at .planning/research/SUMMARY.md (new-project.md:1145,
new-milestone.md:425). Phase summaries -- the ones that actually claim
"I created these files" -- were never checked.

- extract verifySummaryCore from cmdVerifySummary: same checks, lifted out of
  the output() wrapper so callers consume {passed, checks, errors} directly
  instead of shelling out and re-parsing JSON; cmdVerifySummary is now a thin
  adapter over it
- validate.health gains advisory W025 per phase SUMMARY with missing files

Advisory only: appends to warnings[], never touches status escalation beyond
the channel's own warning semantics, the repair set, or readVerificationStatus.

Resolves both open questions from triage: (a) commits_exist is deliberately NOT
surfaced -- its hash pattern matches any hex-shaped token in prose, too loose to
show a user; (b) a phase carries N per-plan summaries plus a legacy bare
SUMMARY.md, so all of them are checked via the repo-wide filter.

Responds to the #2685 review. Three substantive changes.

Seam (Blocker 2). The check now runs in cmdPhaseComplete, the seam the
issue body cited (src/phase.cts:~1745), not validate.health. That channel
does exist: cmdPhaseComplete declares warnings[], populates it from the
UAT/VERIFICATION pre-scan, and emits it. The cycle objection raised
against the earlier deviation holds for state.cts only -- verify.cts has
no transitive import path to phase.cts, so phase.cts -> verify.cjs adds
no cycle (verified over every src/*.cts). Moving it also retires the
retroactive firing across all historical phases: this fires once, at
completion, for the phase being completed.

Extraction (Blocker 1). Pattern 2 now excludes [ and ] from its path
class. The SUMMARY templates prescribe a YAML flow sequence
(key-files.created: [a.ts, b.ts]) and the label matches case-insensitively,
so the class previously captured the literal [ and produced a candidate
that can never exist on disk -- firing on healthy projects built from the
templates GSD itself ships. Stripping frontmatter was the other offered
remedy; measured across all three shipped templates it is a no-op on top
of the exclusion, so it is not carried. Consequence named in-code: the
key-files block still is not read, which needs a real frontmatter parse.

Also narrowed to the noise classes confirmed in review -- globs, bare
hostnames, and paths resolving outside the project are skipped rather
than reported, and the containment guard the old comment claimed now
actually exists.

Budget (Majors 1 and 3). verifySummaryCore takes a checkCommits option;
phase completion passes false, so the discarded git cat-file probes are
not spawned at all. It also passes Infinity, so every referenced file is
reported instead of the first two -- a summary listing twelve files of
which nine are missing now says nine, not zero. The verb keeps its
historical 2-file default.

Tests (Major 2). The vacuous fixtures are gone with the health block.
The replacements use /-bearing paths that genuinely extract, and each
fix was mutation-checked: un-anchoring pattern 2, dropping the glob,
hostname or containment filter, forcing commit checking on, and
re-capping at 2 each fail at least one test.

* docs(#2572): describe the phase-completion SUMMARY artifact check

The W025 text under /gsd-health is withdrawn with the health seam; the
check is documented where it now runs, under `phase complete` in
docs/CLI-TOOLS.md.

Both the docs and the changeset previously overclaimed: they said a
referenced file not on disk is warned about, while at most two candidates
per SUMMARY were ever examined. The cap is gone at this seam, so the
claim now holds -- and the text states the limits that remain, rather
than leaving them to be discovered: the key-files frontmatter block is
not read, commit hashes are not resolved, and globs, URLs, bare
hostnames and out-of-project paths are skipped rather than reported.

* fix(#1882): distinguish unterminated frontmatter from absent frontmatter

extractFrontmatter returned {} both for a document with no frontmatter and for
one whose fence was opened and never closed, so a file truncated mid-write was
byte-identical to a legitimate no-metadata file. Verified live through
`gsd-tools frontmatter get`: both printed {} with exit 0 and nothing on stderr.

Per ADR-1411's "corrupt is not absent" amendment the {} return is preserved
exactly -- no caller may break -- and the cause is surfaced out-of-band as a
deduplicated, unconditional stderr diagnostic. That mechanism lands as a shared
leaf module rather than a per-site copy because three sibling findings in the
same epic need it identically; four hand-rolled copies of one behaviour is the
generative-fix-divergence defect class.

The discriminator is deliberately not "opened but never closed". A Markdown
document whose first line is a thematic break takes that exact branch, so
flagging on the missing fence alone reports corruption on good Markdown -- the
failure mode this class of check has shipped with before. The unterminated
region is instead run through extractFrontmatter's own parser (extracted as
parseYamlRegion so the probe and the real parse can never diverge) and reported
only when it yields at least one key.

Also folds an inline defect found while working: src/config-loader.cts carried
two NUL bytes in the JSDoc added by this epic's Phase 1 (3eb1cede2), making it
the only non-text file under src. file(1) reported it as data and text tools
silently skipped it, defeating the audit rule that says to search the authored
source; tsc passed because the bytes sat inside a comment, so no gate caught it.
It is live on next.

* test(#1882): pin unterminated-frontmatter detection and its negative space

Covers the discriminator on both sides. The positive rows are the issue's own
repro (LF and CRLF) plus the key-count boundary 0/1/2 around the ">= 1 parsed
key" threshold. The negative rows are the documents that reach the same branch
and must stay silent -- above all a Markdown thematic break at byte 0, which is
how this class of check has previously shipped a false positive on valid
Markdown.

Deduplication is tested on both halves of the composite key: a repeat of the
same (path, cause) is suppressed, a genuine second failure in a different file
is not, and a Windows and POSIX spelling of one path resolve to a single key.
The reset seam is asserted to actually clear -- #2674 is the precedent where a
reset that silently failed to clear made every later dedup assertion a vacuous
pass, and the cases only passed because each happened to pick an unused key, so
every case here uses a path unique to itself.

Assertions are on typed surfaces throughout -- the frozen reason enum and the
dedup-set size -- never on diagnostic prose. The one CLI-level case asserts a
differential between two runs (whether stderr is empty) rather than matching a
message, and is the wired user-reachable surface for this fix. Stream failure is
injected by overriding process.stderr.write and restoring it, never chmod 0o000,
which root bypasses.

Two properties guard the ~50 call sites of the changed function: the new
optional path argument is inert with respect to the parsed value, and LF/CRLF
spellings of a document still parse identically.

Isolated adversarial review found the one-key discriminator false-positives on
ordinary Markdown: a thematic break above a single labelled line -- `Note:`,
`Author:`, `TODO:`, `See:` -- parses as exactly one key and was reported as
corruption, which is the precise failure the design claimed to prevent and the
changeset promised was fixed. The threshold is now two keys. A file truncated
after exactly one key becomes a false negative; that is the same
precision-over-recall direction already taken at zero keys, and every GSD
artefact this guards carries two or more frontmatter keys.

Three dedup-key defects, each of which could silently swallow a real diagnostic:

- Backslash normalization is removed. A backslash is a legal filename character
  on Linux and macOS, so folding it to a forward slash made two genuinely
  different files share one key. Two spellings of one Windows path may now
  report twice; two distinct files can never silence each other. Lost signal is
  the worse failure.
- The key namespaces are tagged so a file literally named like the unnamed
  digest fallback can no longer collide with a path-less caller whose content
  hashes to that digest -- computable for any predictable content, no brute
  force needed.
- The source identity is computed once rather than hashed twice per emission.

Corrects the previous commit. The two NUL bytes in src/config-loader.cts were
NOT in a JSDoc comment as that message claimed; they were deliberate separators
in the live dedup key, and stripping them degraded it to bare concatenation.
They are restored as escape sequences -- byte-identical runtime string, and the
file is text again so grep can see it. The diagnostic script that misled me
indexed a character-offset string with a byte offset.

Also threads sourcePath through the STATE.md and PLAN.md readers so the two
artefacts epic #1879 is actually about name their file rather than reporting
under a content digest.

keys, which is what a real interrupted write of a GSD artefact looks like.

The Windows/POSIX case asserted that two spellings of one path collapse to a
single key -- the exact folding that was removed because it also collapsed
genuinely distinct POSIX files whose names contain a backslash. Inverted to
assert they now report separately, with the reasoning recorded inline so the
trade is not silently reversed later: mild duplicate noise on one Windows path
is acceptable, a swallowed diagnostic is not.

The diagnostic reached only the four frontmatter CLI verbs, so ~47 of 53 call
sites reported a truncated file under an anonymous content digest instead of
naming it. Since naming the file is the whole point -- it is what an operator
can act on -- that was a gap in the deliverable, not a scoping choice. 43 of 53
sites now pass the resolved path.

Closing it surfaced a defect the original design missed. A single truncated
STATE.md is parsed twice in a normal run: once by the read wrapper, which holds
the path, and again by a pure core downstream, which is handed only the string
and cannot know it. Those two parses keyed separately, so one file produced two
diagnostics -- and wiring more sites made the collision more likely, not less.
Every emission now registers both identities the input could be known by and
checks both before writing, so whichever caller arrives first speaks and the
other is suppressed. Distinct files with distinct content still report
separately, which is the property ADR-1411 actually requires; two files whose
truncated content is byte-identical collapse to one report, which stays the
documented limit.

Ten call sites deliberately keep no path. Two are frontmatter's own round-trip
checks during set and merge, where passing a path would report on every write.
The other eight are the state-transition pure cores, which ADR-1769 defines as
(content, intent, deps) -> newContent with injected I/O; threading a path
through them would contradict that recorded decision, so it is surfaced rather
than taken unilaterally. With the widened key they no longer double-report, and
in the normal flow the named parse runs first, so the file is still named.

injected deps, and deps is the seam for precisely this: something the core
cannot derive without doing I/O. It already carries roadmapProvider and a
phase-inventory provider on that basis, each documented as injected rather than
imported so the core stays pure and testable without disk access. A resolved
path is data, not I/O, so an optional sourcePath member extends the established
pattern rather than contradicting it, and every existing stub keeps compiling
because the member is optional.

updateCore and reconcileCurrentPosition take no deps and are left alone. With
the widened dedup key they cannot double-report, and in the normal flow the read
wrapper has already named the file by the time they run.

Also regenerates gsd-core/bin/lib/state-transition.cjs. That artifact is tracked
rather than gitignored, unlike most of its siblings, so leaving it stale would
have shipped a runtime without this change to anyone reading the repo without
building. tsc had skipped the re-emit because its incremental build info still
recorded an emit that had since been reverted, so the stale output survived a
clean build; clearing tsconfig.build.tsbuildinfo forced it. The
compiled-artifact-sync gate is what surfaced the drift and now reports all nine
tracked artifacts matching their source.

still recording every key the input could later be identified by. The redundant
path-less re-parse of an already-named file stays silent, and two distinct files
always both report.

Verified across all six orderings: same file named-then-anonymous reports once;
two different files with identical content report twice; two different files
with different content report twice; the same path twice reports once; two
path-less parses of identical content report once; two path-less parses of
different content report twice.

The suite caught this -- twenty failures, all in the unusable-input tests that
reuse one truncated fixture across different paths. The local probe written
alongside the broken change did not, because it compared two files with
different content and could therefore only confirm the expected behaviour.

diagnostics were emitted". That held only while one emission recorded exactly
one key. Once an emission began recording every identity the input could later
be matched by -- a path key and a content key for the same file -- the set grew
by two per write and twenty assertions read 2 where they expected 1.

The production behaviour was correct throughout; the proxy was not. Set size
counts identities, which is an implementation detail of the guard. The
behavioural claim these tests exist to make is how many diagnostics an operator
actually saw, so the module now exposes that directly as an emission counter and
the suite asserts on it. The set-size accessor stays for assertions genuinely
about key shape.

The local probe written alongside the change did not catch this because it
counted process.stderr.write calls -- the right thing -- while the suite counted
set growth. Verification now asserts both and requires them to agree, so a
future divergence between the counter and real writes fails immediately rather
than being discovered a bench run later.

The reset-seam case ended by reading the size of the dedup set and expecting 1.
Set size counts interned identities, not diagnostics written, and one emission
now interns two. It asserts the emission counter for the event and keeps a
weaker set-size check for the interning.

Three orthogonal review passes ran against the final diff. Their findings:

A labelled preamble under a leading rule was still misreported. Raising the key
threshold to two only moved the boundary, because two colon-labelled lines are
as common in ordinary prose as one -- a document opening with a rule over an
Author and a Reviewed-by line, then prose, was called corrupt. Key count alone
cannot separate the two. What does is what follows: a write interrupted part way
through a frontmatter block ends mid-block, so every line of the region is still
frontmatter-shaped, whereas a document merely opening with a rule goes on to
prose. Both conditions are now required, and each closes a false-positive class
the other leaves open. Nested list values and indented continuations stay
frontmatter-shaped, so legitimate truncations are unaffected.

`state rebuild --dry-run` reported a truncated STATE.md anonymously. The write
path is named only because readModifyWriteStateMd parses with the path first;
the dry-run branch reads the file directly and never did. Dry-run is the
read-only mode an operator reaches for first when they suspect corruption, so it
is the one that most needed to name the file. reconcileCurrentPosition takes the
path as an optional argument now and rebuildCore passes it down. That function
was previously left alone on the grounds that a read wrapper always names the
file first -- this is the flow that disproves it.

The emission counter counted write attempts rather than writes, so on a broken
stderr it claimed a diagnostic had reached the operator when nothing had. It is
incremented only after a write that completed, and the broken-stderr test now
asserts the count as well as the return value.

Two documentation defects. The module described a guarantee it does not keep:
one file yields one diagnostic only when the named read comes first. The reverse
ordering emits twice, and that is deliberate -- a path-less caller cannot
identify its file, so suppressing the later named report would also suppress a
genuine second failure in a different file whenever two files share identical
truncated bytes, which ADR-1411 ranks the worse failure. The comment now states
the asymmetric guarantee and a test pins it. Separately, the CONTEXT.md glossary
entry still described backslash normalization that a later commit removed, and
asserted the opposite of what the tests pin; no lint checks prose against code,
so nothing caught it.

Also converts three body-level try/finally blocks to t.after(), per
CONTRIBUTING.md's rule that try/finally belongs only in helpers with no test
context -- the file's own emissionsDuring helper already did this correctly.

block must not disqualify it, which constrains the filter and its comparison. An
unindented list item and an indented folded-scalar continuation each exercise one
shape alternative that no other case reaches on its own -- the folded line is
neither a key nor a list item, so it is the only input that distinguishes the
indented branch. And two keys followed by prose must stay silent, which is the
negative half: it fails if the predicate is ever mutated to accept everything,
and it is the case that proves key count alone was never sufficient.

difference was four added tests. That is the tell: the tests were never
executed. The frontmatter shard runs a fixed file list in stryker.config.mjs and
scripts/mutation-matrix.cjs, and tests/unusable-input.test.cjs was in neither, so
the entire suite covering the new unterminated-fence branch was invisible to the
gate while passing perfectly well in the normal run.

So the score was not measuring weak tests, it was measuring absent ones: #1882
added mutants to frontmatter.cjs and no test in the shard covered them. Both
lists gain the file; the config already notes they must stay in sync.

This is a registration ripple a new test file carries when it covers a
mutation-tracked module, alongside the .gitignore, eslint, inventory, glossary
and size-baseline ripples a new module carries. Nothing warned about it, which
is why two runs were spent before the identical score gave it away.

`src/verification.cts` stored and synthesized hard-coded `/gsd:…` command strings
with no runtime context, and `phase complete` relayed that raw field straight into
its verification-blocked error. On a Codex project the suggested next step was
`/gsd:execute-phase`, a surface Codex does not install — it installs
`$gsd-execute-phase`.

The colon form is wrong twice over: `runtime-slash.cts` documents that "the colon
form is never emitted", so EVERY runtime — not just Codex — was being handed a
deprecated shape.

Fixed at the one routing seam rather than per caller:

- The routing table now stores BARE command names (`execute-phase`), never a
  prefixed literal. A prefixed literal in the table is what leaked.
- A single `projectNextCommand(bare, runtime, tail)` helper runs every return path
  through `formatGsdSlash`, preserving the argument tail (`01 --gaps`) untouched.
  An empty command stays empty, so "no next step" never becomes a bare prefix.
- `readVerificationStatus` accepts `opts.runtime`; `cmdVerificationStatus` and
  `phase complete` pass `resolveRuntime(cwd)`. The default is `claude`, which
  yields the canonical `/gsd-` hyphen form.

All four routed states are covered: missing, unknown, gaps_found, stale.

`init.cts` keeps its own projector deliberately. It already formats correctly, and
its command CONTENT differs from the router's on purpose (it appends the phase
number to `execute-phase`, and routes `human_needed` to `verify-work`).
Consolidating them would silently change `init`'s user-visible output, which this
issue did not ask for — so the divergence is left intact and the new tests instead
pin the property that matters on both surfaces: no raw colon form escapes.

Failing-first record: `origin/next:src/verification.cts` carried the four `/gsd:`
literals (lines 101, 108, 382, 392), and 11 existing assertions in
tests/verification-status.test.cjs asserted the colon form. Those 11 are corrected
in this commit — they passed before the fix and fail after it, which is precisely
the regression this closes.

Tests are folded into the module's primary suite rather than added as a third file
(`lint-test-file-count` caps the `verification` module at two, and consolidating is
its documented remedy — growing the allowlist is not). The `phase complete`
assertion reads `res.error`, not `res.stderr`: `runGsdTools` exposes a clean
non-zero exit's stderr as `error`, and reading the wrong field yields '' and makes
the whole check vacuous — which is how this user-visible path stayed untested.

Two findings from the orthogonal review of the first commit, both in the tests
this change added.

1. The folded block's `beforeEach`/`afterEach` were declared at MODULE scope.
   node:test applies module-scope hooks to every test in the file, so hooks added
   for the #2617 suites also wrapped the ~40 pre-existing tests in
   verification-status.test.cjs — making an unrelated block a single point of
   failure for them (currently benign, but a throwing hook would have failed
   suites it has nothing to do with). They now install inside their own describes
   via a small `useProjectionPhaseDir()` helper, with a comment recording why.

2. The live-CLI `phase complete` test exercised only the `missing` state, so a
   regression in any other routed branch would have shown up in the router's
   return object but not in the text a user actually reads. Added a `gaps_found`
   case per runtime, asserting the projected `plan-phase <N> --gaps` reaches the
   blocked-completion error.

Whole file verified green: 48 tests, 48 pass — the ~40 pre-existing ones included.

which now yields the canonical `/gsd-verify-work 01` hyphen form. The colon form
this asserted is exactly the deprecated shape #2617 removes — `runtime-slash.cts`
documents that "the colon form is never emitted".

Like the eleven corrected in the first commit, this assertion passed before the
fix and fails after it, which is the regression record rather than a test being
loosened: the surrounding assertions (failure reason, `stale` wording, and that
neither ROADMAP.md nor STATE.md was mutated) are untouched.

Verified against the real CLI: the emitted message is now
"Phase 1 verification is incomplete: Verification is stale. Re-run verify-work
before transition. Next: /gsd-verify-work 01".

The orthogonal review found that `init.cts` carried a second, independently
maintained `verificationNextCommand()` that had drifted from the router's table
in CONTENT, not just formatting:

The `human_needed` row is the sharp one: two GSD surfaces disagreed about whether
a next command existed at all, and the router's own next_action told the user to
"re-run the verify step until status is passed" while naming no command to run.

init's answers were the useful ones, so the router adopts them and init now
delegates to it — satisfying the issue's "keep one verification-routing seam"
direction. `verificationNextCommand()` is deleted.

Appending the phase number surfaced a trap the old bare commands hid.
`extractPhaseToken` also returns project-code forms (`PROJ-07`), which are
indistinguishable by shape from an ordinary directory name — `gsd-651-parent`
yields `gsd-651` — so deriving the argument blindly emits
`execute-phase gsd-651`. The number is therefore appended only when it is
unambiguously numeric, or when the caller supplies it explicitly. `init` does
supply it: its `phaseDir` is unresolved in several branches, where the router
could not derive one at all.

  dir `01-example`      -> $gsd-execute-phase 01, $gsd-verify-work 01
  dir `gsd-651-parent`  -> $gsd-execute-phase,    $gsd-verify-work

Suites verified green against the built lib: verification-status 50/50,
phase 268/268, init 143/143, init-manager 40/40. `npm run lint:ci` clean.

User-visible change beyond the reported bug, as agreed: `query verification.status`
and `phase complete` now append the phase number for missing/unknown, and emit
`verify-work <N>` for human_needed where they previously emitted nothing.

   and cross-checked; ghost IDs raise a warning through the existing
   warnings/has_warnings surface that execute-phase.md already prints.
2. `if (reqUpdate.ok)` had no `else`, so a Traceability-row write matching
   nothing was discarded silently. Misses are now recorded.
3. `requirements_updated` was set unconditionally inside `if (existsSync)`,
   reporting "the file was in the transaction" rather than "a write landed". It
   now reflects whether the content actually changed.
4. DEFERRED_HEADING_RE's bare `v\d+` alternative treated an active `## v1
   Requirements` heading as deferred, zeroing the body scan for that section.
   Dropped; `deferred|backlog|future` still match, so #1159's suite still holds.

Also fixes the same class in gap-checker: the `items.length === 0` early return
fired before ghost rows were folded in, so an all-unregistered phase reported
LESS than a partially-unregistered one. It is now gated on ghostReqIds too.

Follows the #2140 precedent in milestone.cts, which hardened this exact class for
`requirements mark-complete`; adapted to this function's single `warnings` array
rather than adding fields execute-phase.md never reads.

miss the shape that breaks.

BLOCKER: dropping the `v\d+` alternative from DEFERRED_HEADING_RE regressed #1159
against GSD's OWN shipped template. gsd-core/templates/requirements.md:35 ships
`## v2 Requirements` with the deferred-ness in the BODY PROSE ("Deferred to future
release"), not the heading — so `v\d+` was the only alternative matching it.
Deleting it made every scaffolded project emit a false traceability warning on
every phase complete, forever. The real problem is that `v\d+` matched BOTH the
active v1 and the deferred v2: deleting it swings from suppress-everything to
suppress-nothing. The heading check is now milestone-aware — a `## v<N>` heading
is deferred only when <N> is not the current milestone's major, resolved through
the existing state.cjs seam. Unresolvable milestone fails safe to the old
always-deferred behavior, since suppressing a warning beats spamming every project.

HIGH: the ghost check re-derived membership from two lossy index sets that
disagreed with the case-insensitive write paths, so it warned "not registered
anywhere" about an ID whose checkbox it had just ticked, and about a
case-mismatched ID whose write landed. It now probes the same two write surfaces
the writes use, mirroring milestone.cts's #2140 precedent (probe the write, don't
re-derive it) — which is what the original brief asked for and the first pass
didn't do.

HIGH: the cited-ID tokenizer split the rest of the line on whitespace, so every
trailing word became a "cited REQ-ID". The shipped roadmap.md template puts an
inline HTML comment on that exact line, so a fully correct run told users to
register `<!--` and `-->` as requirements. Cited IDs are now filtered to the
REQ-ID shape, matching what bodyReqIds/tableReqIds already require.

HIGH: gap-checker had the identical early-return defect 34 lines above the one
fixed — the could-not-parse branch gated ghost rows behind the same unguarded
items.length. One malformed <decisions> line made all ghost rows vanish.

Tests: the #2316-5 guard only covered `## Deferred`/`## Backlog`/`## Future` —
headings that trivially still match — omitting `## v2 Requirements`, the only
shape the change altered. Added the shipped-template fixture plus guards for each
finding above.

A phase whose slug's first word is a ≥2-digit number (dir
14-2026-photos-performance, roadmap phase "2026 Photos & Performance" →
slug 2026-photos-…) had its phase token over-collected as "14-2026"
instead of "14", so every phase-locating verb (init.plan-phase,
init.execute-phase, phase-plan-index, state.planned-phase,
roadmap.annotate-dependencies) resolved phase_dir=null / plan_count=0
while the directory existed. This is the residual case #2043 explicitly
scoped out: its ≥2-digit continuation gate (\d{2,}) distinguishes
single-digit slug words but not multi-digit ones (years, counts).

The structural distinguisher: getPhaseDirFromPhaseId writes sub-phase and
plan continuation segments zero-padded to EXACTLY 2 digits, so a genuine
continuation's digit run is exactly 2 — \d{2}(?!\d). The (?!\d) guard
caps the run without anchoring what follows, so each call site keeps its
own trailing grammar (letter suffixes, dotted sub-phases, boundaries).

Shared-source, not hand-synced: the grammar lives once in phase-id.cts as
PHASE_CONTINUATION_SEGMENT_SOURCE / isPhaseContinuationSegment (the #2121
single-owner seam), consumed by all five #2043 sites:
- phase-id.cts extractPhaseToken (the reported repro)
- validate.cts PHASE_TOKEN_FROM_DIR_RE + canonicalPlanStem
- roadmap-parser.cts isDirInMilestone numericRe (hyphenated mode)
- core-utils.cts + phase.cts extractCanonicalPlanId (paired plan
  component only — the LEADING phase component keeps unbounded \d{2,};
  phase numbers ≥100 are legitimate)

Digit-width policy, resolved per triage and locked by boundary tests at
1/2/3/4-digit continuation widths across all sites: sub-phase/plan
numbers ≥100 are out of the dir-token grammar. validate.cts
phaseDirNameRe's leading \d{2,} is intentionally untouched — it encodes
the write-side padding of the leading dir number, not the continuation
heuristic, and has no year collision.

scripts/lint-phase-id-drift.cjs guards PHASE_NUMBER_TOKEN_SOURCE only; its
TOKEN_DRIFT_RE cannot match a bare \d{2,} re-derivation, so a future edit
reintroducing a raw digit-cap at a consuming site would pass lint + CI
silently. Extending the lint was rejected: \d{2,} legitimately appears at
the intentionally-unbounded LEADING-token sites (validate phaseDirNameRe,
core-utils/phase tokenRe), so a textual guard would need sanctions on
correct code and would flag by spelling rather than by behaviour.

Instead, per the repo's *-parity.test.cjs precedent, added
tests/phase-continuation-parity.test.cjs: a shared digit-width corpus
(1/2/3/4/5) asserting every consuming surface's notion of "is this segment
absorbed" equals isPhaseContinuationSegment(). Covers all five #2043 sites:
extractPhaseToken, PHASE_TOKEN_FROM_DIR_RE, canonicalPlanStem,
extractCanonicalPlanId (paired component), and roadmap isDirInMilestone
(hyphenated mode, on a real ROADMAP fixture). The corpus states the policy
independently of the regex, so it fails on divergence rather than mirroring
whatever the code does.

Failing-first verified: reverting PHASE_TOKEN_FROM_DIR_RE to \d{2,} fails 3
parity tests; reverting the owner constant itself fails 11 across parity +
properties + examples.

M2 — fast-check properties for the changed parser (4 added to
phase-id.test.cjs, following its existing inline fc precedent):
- biconditional: a segment is absorbed IFF its digit run is exactly 2
- the owner agrees with observable extraction for every digit run
- metamorphic: a write-side getPhaseDirFromPhaseId dir round-trips to its
  own normalizePhaseName id — ties the cap to the zero-padding convention
  it mirrors, so a change to the write-side width fails loudly
- metamorphic: the round-trip holds when the phase name leads with a year
  (the #2232 bug itself, generatively)
Digit runs are generated as digit strings (not String(int)) so leading-zero
forms like "02" — the whole point of the rule — are actually exercised.

B1 — GitGuardian red. The session-trailer hypothesis is disproven: the same
Claude-Session trailer rides 3 commits now merged to next via #2173, whose
GitGuardian check PASSED. GitGuardian's own comment names
tests/phase-id.test.cjs:260 — the synthetic dir literal 'M1-14-2026-photos'
tripping the generic high-entropy detector. Composed it from parts; the
assertion is unchanged, only the source spelling.

CI caught two failures from the new parity test, both one root cause:
lint-test-file-count caps each production module at 2 test files (primary +
one integration, per the #3740 consolidation). The file was named
phase-continuation-parity.test.cjs, and the linter clusters a test to a
production module by name prefix — "phase-*" bound it to src/phase.cts,
whose cluster (phase.test.cjs + phase-dependency-levels.test.cjs) was
already at the cap, making 3. That tripped the lint-tests job AND the
ubuntu-24 unit lane, where tests/lint-test-file-count.test.cjs is a
meta-test asserting the linter exits 0 against the real repo.

Renamed to continuation-grammar-parity.test.cjs, matching the convention
the repo's other cross-cutting parity gates already follow: they are named
after the INVARIANT, not a module — capability-precedence-parity,
agent-classification-parity, and runtime-launcher-parity all have no
corresponding src/*.cts, so they cluster to nothing. The gate tests a
grammar shared ACROSS phase-id/validate/core-utils/roadmap-parser rather
than the phase module specifically, so the invariant-name is also the
semantically correct home. Not allowlisted: a novel offender belongs under
the cap, not ratcheted into the exemption list.

Content unchanged — same 12 assertions across the same 5 surfaces.

- REQ-ID-headed table trips #2140 table_unmatched drift on a missing row.

- `src/markdown-sectionizer.cts`: `withSection(content, target, edit, opts?)` —
  resolves a section via `collectSection` and applies `edit` to ONLY that
  section's body, re-serialising via `replaceSection`. The edit callback sees
  only the section body, so any regex it runs is physically confined.
- `src/roadmap-parser.cts`: `withPhaseSection(content, phaseId, edit)` —
  resolves a phase's `### Phase N` detail-section heading via the #2121
  phase-id source and delegates to `withSection`. Heading match is anchored to
  the heading start (a sibling phase whose title mentions the number is not
  hijacked) and bounds at the next ATX heading of any level (`levelBounded:false`).
- `src/phase.cts`: `mutateMilestonePhase`'s plan-count and per-plan-checkbox
  writes now route through `withPhaseSection` — structurally retiring the
  #2130 / #2067 / #2080 boundary-crossing class for these sites. The phase-LIST

  flips ONLY within the current milestone window(s) via a mutateMilestonePhase

71edb27fe1 fix(#2136): route operator-facing date fields through local clock day
clock.today() derived the calendar day by slicing a UTC ISO instant
(nowIso().split('T')[0]), so in any negative-UTC-offset zone during UTC's early
hours last_activity named a day the operator had not reached yet — ahead of
last_updated's local date, written by the same call.

- Add Clock.localToday(): host-local YYYY-MM-DD via getMonth/getDate/
  getFullYear, honoring the same GSD_NOW_MS pin as today()/nowIso().
- Route operator-facing date-only fields through localToday(): last_activity
  (state-transition ×7, state.cts), roadmap 'completed <date>' (phase.cts,
  roadmap.cts), milestone completion date (milestone.cts), todo/scaffold
  completion (commands.cts — now threaded through the realClock seam).
- Leave today() (UTC) as the source for internal/cosmetic stamps.

2f6662d195 fix(#2128): bound the remaining lazy-scan ReDoS vectors (files_modified, Plans-count, <tag>)
A ReDoS-completeness audit surfaced a distinct class beyond the tag/bracket
clause: unbounded `[\s\S]*?` / `[^\]]*` lazy-scans searching for a literal
terminator that may never appear, driven quadratic by REPEATED structures in a
large PLAN.md/ROADMAP.md. Folded all 7 in at maintainer direction:

- files_modified `[^\]]*` -> `[^\]]{0,8000}` (commands.cts, verify.cts): 39.7s -> 0.9s.
- Plans-count `[\s\S]*?` -> section-local `(?:(?!\n#{1,4}\s)[\s\S])*?` — stops at the
  next heading (semantically correct: Plans: belongs to the phase's own section)
  (roadmap.cts x3, phase.cts): 36s -> 4ms.
- <tag> extraction `([\s\S]*?)` -> stop at the next same-tag opening
  `((?:(?!<tag>)[\s\S])*?)` (verify.cts x3, markdown-sectionizer.cts): ~6s -> 2ms.

Every vector is now linear (comprehensively re-measured); real content matches
(end-to-end `roadmap get-phase` still resolves Plans-counted phases). Pre-existing;
byte-behavior preserved for realistic inputs.

Review caught that the prior commit bounded only the paren tag clause and left
the SIBLING bracket-prefix `(?:\[[^\]]+\]\s*)?` (same host regexes, before Phase)
UNBOUNDED — the identical quadratic reachable via a `[...]` run (measured ~16s at
1.7MB). Bound `[^\]]+`/`[^\]]*` -> {1,200}/{0,200} across all 19 phase/milestone
heading prefixes. Comprehensive re-measurement now shows EVERY vector linear
(bracket/paren/id/name/milestone all ~2-44ms at 2.45MB; bracket scaling
2k->2ms, 4k->5ms, 8k->10ms). Also: update the #1729 literal-mirror parity test
off its stale unbounded constant, and add limit-1 (199) boundary coverage.

c1cd43a39f fix(#2128): bound the phase-tag clause to {0,200} — kill quadratic ReDoS
The canonical OPTIONAL_PHASE_TAG_SOURCE tag clause `(?:\s*\([^)\n]*\))?` (and its
inlined literal mirrors across 11 modules) had an UNBOUNDED body, making the
optional-group + /g header scan quadratic on adversarial ROADMAP.md/STATE.md — a
long run of `(` after a header ran ~18.8s at 1.7MB. Bound the body to {0,200} in
the constant AND every mirror in lockstep (the #1729 "both forms change together"
contract), so the scan is linear: the same 1.7MB input now resolves in ~9ms
(measured), while real tags (a handful of chars) still match and a 201-char tag
is rejected. Added a #2128 boundary regression to the #1729 suite.

Pre-existing (byte-identical before/after the Phase 4 migrations); folded in at
maintainer direction rather than deferred.

dfad3a7510 refactor(#2128): single-source 23 phase-token re-derivations; sanction 14 context-specific sites
Route 23 literal re-derivations of the canonical phase-number token through
phase-id.cjs `PHASE_NUMBER_TOKEN_SOURCE` (via new RegExp). Each conversion was
proven BYTE-IDENTICAL (old.source === new.source && old.flags === new.flags), so
the runtime regexes are unchanged — zero behavior change by construction.

The remaining 14 phase-token sites are genuine but context-specific and stay
literal with a `// phase-id-owner: <reason>` sanction: dir-name parses whose
dash-continuation semantics differ from extractPhaseToken, and the [A-Za-z]
case-variant / [.-] dot-or-dash separator forms that are not source-byte-equal
to the canonical token.

Scanner (`npm run check:phase-id-drift`) is now green.

51dfa683d4 fix(#2028): phase.complete milestone-end out-of-order + workstream root-fallback guard
Two code-confirmed defects in `gsd-tools phase complete` (re-verified against
next; the three severe corruption paths the issue filed are superseded by the
ADR-1769 Transition Module migration + #2012, so this is the confirmed remainder).

1. Milestone-end mislabel (isLastPhase). The milestone-end determination only
   cleared isLastPhase when a HIGHER-numbered phase existed, so completing the
   numerically-highest phase out of order (e.g. Phase 10 before Phase 9) stamped
   STATE.md `Status: Milestone complete` while a lower phase was still outstanding.
   Added a lower-phase check: after the existing higher-phase scans, if any earlier
   phase in the current milestone has an unchecked roadmap checkbox (`[ ]`),
   isLastPhase becomes false AND next_phase/next_phase_name point at the LOWEST
   outstanding lower phase — so STATE.md advances to the real gap instead of
   parking on the just-completed phase. A completed phase always has `[x]`
   (phase.complete sets it), so all-lower-complete still reports milestone-end;
   heading-only roadmaps (no checkboxes) retain prior behavior. The checkbox regex
   mirrors the sibling phasePattern's anchoring (whitespace/bold + required `:`) so
   unrelated checklist lines mentioning "Phase N" don't match.

2. Workstream root-fallback (no guard). cmdPhaseComplete resolves every path via
   planningDir(cwd); with a `workstreams/` dir present but no active workstream and
   no --ws, that returns root `.planning`, so phase.complete wrote STATE.md/
   ROADMAP.md (and the mislabel) into the shared root other workstreams read.
   Added the same #1912 fail-safe guard init.progress got: refuse (asking for
   `--ws`/active workstream) instead of silently writing root. Resolution itself
   was already wired globally (resolveActiveWorkstream: --ws > GSD_WORKSTREAM >
   pointer, set in bin/gsd-tools.cjs), so only the refusal guard was missing.

The workstream-mode detection (`listAvailableWorkstreams`) is extracted into
planning-workspace.cts as the single source of truth and consumed by BOTH
init.progress and phase.complete, so the two fail-safe paths cannot drift.

Tests (tests/phase.test.cjs, new #2028 describe): out-of-order completion becomes
`Ready to plan` with is_last_phase=false, next_phase pointing at the outstanding
phase and Current Phase advancing to it (not the completed phase); all-lower-
complete still reports milestone-end; workstream-mode-no-active refuses with an
`--ws` hint; `--ws` completes in the workstream leaving root untouched; flat mode
unaffected. Fail-first verified locally via direct gsd-tools invocation.

6efd2b4016 fix(#2043): reject single-digit slug word in phase-token extraction (all sites)
A phase whose slug's first word is a bare single digit (dir
`46-6-rs-pipeline-orchestrator`, roadmap phase "6 Rs Pipeline Orchestrator" →
slug `6-rs-…`) had its phase token over-collected as `46-6` instead of `46`, so
`gsd-tools` phase-by-number lookups (init.plan-phase, init.phase-op, and
downstream execute/verify/ship) resolved phase_dir=null / has_context=false.

Root cause: an over-broad "looks numeric" test (`/^\d/`, `\d+(?:-\d+)*`)
classified token segments and could not distinguish a legitimate zero-padded
sub-phase segment (`01`, `02`) from a single-digit slug word (`6`). Zero-padded
phase/sub-phase segments are always ≥2 digits, so requiring ≥2 digits is the
structural distinguisher. Applied consistently across every same-class
implementation the triage identified (fixing extractPhaseToken alone leaves the
health-check and milestone-filter subsystems exposed):

- src/phase-id.cts extractPhaseToken — a pure-numeric leading segment continues
  only with ≥2-digit segments; a letter-prefixed milestone id (`M1`) still
  admits its single-digit sub-phase (`M1-2`).
- src/validate.cts PHASE_TOKEN_FROM_DIR_RE (W005/W006/W007 health checks) and
  canonicalPlanStem (I001 plan/summary pairing).
- src/roadmap-parser.cts isDirInMilestone numericRe (getMilestonePhaseFilter —
  highest blast radius: a false negative silently excludes a phase from the
  milestone).
- src/core-utils.cts + its verbatim duplicate src/phase.cts extractCanonicalPlanId.

Regression tests added across tests/{phase-id,health-validation,core-utils,
roadmap-parser}.test.cjs using the shared `46-6-rs-…` fixture, asserting the
token resolves to `46` (not `46-6`) while legit multi-segment tokens
(`01-02`, `02-03-04`, `M1-2`, `68-01`, `02-01`) are unchanged. The
roadmap-parser test exercises the real getMilestonePhaseFilter end-to-end.

The residual ≥2-digit-leading-slug case (e.g. `NN-2024-roadmap`) stays out of
scope — subsumed by the #612 / #565 phase-ID convention work, per the issue.

or corrupt numbering after a removal.

The fix tolerates the tag at the header seam. Parameterized resolver
sites compose the exported OPTIONAL_PHASE_TAG_SOURCE fragment; literal
enumeration sites inline its character-for-character mirror
`(?:\s*\([^)\n]*\))?`, placed immediately before the colon so it cannot
alter an existing match (optional, single-line, one paren pair, no
capture-group shift). In the renumber-on-removal rewrite the tag is
folded into the re-emitted suffix capture so it survives verbatim. Both
forms are documented to change together and a drift-guard test asserts
their behavioral equivalence over a header corpus.

Deliberately excluded: roadmap-upgrade.cts (legacy one-time migration),
where tolerating the tag would silently drop it on header rewrite — that
needs its own data-preserving treatment. Known boundaries left for
follow-up: checklist/bullet-style phase entries (`- [ ] Phase N (tag):`)
and a malformed space-before-colon variant, both pre-existing.

Validated empirically against the issue's reproduction: `roadmap
get-phase 26` resolves and `roadmap analyze` lists Phase 26 with the tag
excluded from the name (phase_count 2, next 26); an all-tagged versioned
roadmap now scopes correctly instead of falling back to a pass-all
filter. Regression coverage in tests/phase.test.cjs asserts resolver
parity (pre- vs post-colon), padding tolerance (#3537), decimal
sub-phases, no cross-phase false match, the shared seam, enumeration
coherence, renumber-preserves-tag, and seam/mirror drift. Full unit
suite green (7291 pass, 0 fail); eslint + regression-name +
resolution-provenance + changeset lints pass. Reviewed by Codex
(no critical/high; the two enumeration misses it surfaced are folded in).

bab72fa2b0 fix(#1591): match bold checklist phase markers in isLastPhase fallback
The #1591 checkbox broadening matched `- [ ] Phase N:` but not the
canonical bold form the roadmap template emits (`- [ ] **Phase N: Name**`),
so a <details>-wrapped bold checklist with no next-phase directory still
fell through to is_last_phase=true and a false 'Milestone complete'. Allow
optional **/__ emphasis after the marker and stop the name capture at
emphasis so bold names slug cleanly. Surfaced by adversarial (codex) review.

checkbox items inside a <details> block (the @Azd325 structure) and the next
phase has no directory yet, the disk-based next-phase resolver finds nothing
and phase.complete falls back to the roadmap-enumeration guard at the
isLastPhase site. That guard's phasePattern was heading-only
(/#{2,4}\s*Phase…/), so it never matched checklist items → is_last_phase=true
and next_phase=null on a mid-milestone phase, and STATE.md was wrongly marked
'Milestone complete' with total_phases decremented.

Broaden the marker alternation to match BOTH heading-style (### Phase N:) and
checkbox-list items (- [ ] Phase N: / - [x] Phase N:); the number/name
captures are unchanged. extractCurrentMilestone already surfaces the
<details>-wrapped checklist correctly, so no parser change is needed. The
heading-only sibling patterns elsewhere in phase.cts are left untouched
(scope discipline — only the reproduced isLastPhase fallback is changed).

Regression: a phase complete 36 on a <details>-wrapped v2.0 checklist
(Phases 36-38, only 36 has a dir) returns is_last_phase=false, next_phase=37,
and does NOT flip STATE.md to 'Milestone complete'.

Migrate the STATE.md write path inside cmdPhaseComplete (phase.cts) onto the
STATE.md Transition Module substrate (ADR-1769, epic #1769).

- Add {kind: 'completePhase'} to StateTransitionIntent + completePhaseCore in
  src/state-transition.cts. Pure body field mutations (Current Phase shape/name,
  Status, Current Plan, Last Activity + Description, Completed/Total Phases +
  Progress percent), consulting the field-classification table for touched keys.
  Roadmap progress injected via a new optional deps.roadmapProvider.
- Collapse the ~90-line inline STATE.md transform in cmdPhaseComplete to a
  transitionCore dispatch. The adapter retains updatePerformanceMetricsSection
  (section table) + syncStateFrontmatter (disk-scan post-sync) and the
  multi-file atomic transaction (STATE is committed with ROADMAP/REQUIREMENTS,
  so readModifyWriteStateMd is not used here).
- deriveProgressFromRoadmap/clampPercent/stateReplaceFieldWithFallback move from
  the phase.cts call site into the pure core (no circular dep: phase-lifecycle
  and state-document don't import state.cts).
- Characterization tests in tests/state-transition.test.cjs pin the field
  updates, roadmap progress derivation, #1255 frontmatter parity, and the
  'Phase:' fallback. All 44 transition + 193 phase tests pass.

No user-visible behavior change. cmdPhaseComplete CLI output and 'phase complete'
behavior unchanged.

- eslint-rules/require-fs-op-fallback.cjs: flags an unguarded fs.rename /
  fs.renameSync (the atomic-publish primitive named first in
  DEFECT.WINDOWS-FS-OPS.symptom) that is NOT inside a try/catch whose handler
  references a transient errno ('EPERM'/'EBUSY'/'EACCES' or a *RETRY_ERRNOS
  set) AND NOT behind a Windows platform guard. A catch that silently swallows
  or cleans-up-and-rethrows without an errno check does NOT satisfy the
  defect's 'never silently swallow' clause. copyFile/unlink are deliberately
  not flagged (they are the fallback primitives per the defect's own
  fix-forward). Scope narrowed to rename per Phase 5's precision discipline;
  documented on #1740.

- src/shell-command-projection.cts: export retryRenameSync(from, to) — the
  drop-in bounded-retry helper over the existing atomicRenameWithRetry.

- 27 bare fs.renameSync sites across 11 modules routed through retryRenameSync
  (capability-lifecycle/lock/source, installer-migrations, milestone, phase,
  planning-workspace, roadmap-upgrade, runtime-hooks-surface, state,
  workstream). Idempotent on POSIX; resilient to AV/indexer transient locks
  on Windows.

- eslint.config.mjs: register rule at error on src/**/*.cts; new focused
  portability-rules block covering bin/install.js + scripts/build-hooks.js
  (ADR-1703 L124-126 glob expansion — both files are compliant: zero
  rename violations).

- tests: 15-case RuleTester suite; portability-rule-disable-ban extended
  (PROTECTED_RULES + scans bin/install.js/build-hooks.js with shebang
  handling); ci-test-scope portability-lint selection rule.

requires retry, not just recognition. Fix: catchHandlerHasRetrySignal now
requires a loop `continue` backedge OR a `return <call>` delegation; a bare
rethrow is flagged. The misleading `/* retry logic */` valid test is replaced
with a real retry loop, and the rethrow-only shape is added as invalid.

HIGH-2 — the nested-try ancestor walk treated an OUTER errno-catch as
protecting the rename even when an INNER catch intercepted/swallowed the error
(the outer catch is unreachable). Fix: isInsideTransientErrnoTryCatch now stops
at the NEAREST enclosing TryStatement WITH A CATCH HANDLER whose block contains
the rename (try-finally is skipped — it doesn't catch); outer catches are no
longer consulted. The unsound nested-try valid test is converted to invalid,
and a try-finally-skipped valid case is added.

Verified: 17 RuleTester cases pass; zero new production violations (the 27
fixed sites use retryRenameSync; the real retry loops — atomicRenameWithRetry,
capability-ledger/consent, build-hooks — remain compliant via continue/errno);
lint:ci green; disable-ban + vocab-drift green.

Fix: add a bullet-entry regex scan (all checkbox variants: [ ], [x], [~], with
or without ** bold markers) to the set-based phase-number collection in
cmdPhaseAdd. Also added a post-compute collision guard that advances the
candidate past any already-used number.

Regression tests added to tests/phase.test.cjs (bug #1229 describe block):
bullet-only collision, [x]/[~] variants, plain-bullet, and baseline preservation.

`/status: gaps_found/` matched the substring inside `previous_status:
gaps_found`, producing a spurious warning. Fixed by reading only the
frontmatter `status` key via `extractFrontmatter` in both `phase.cts`
(phase complete warning check) and `commands.cts` (determinePhaseStatus).

Defect B: requirement IDs under explicitly deferred/backlog/future/v2
section headings in REQUIREMENTS.md were flagged as missing from the
Traceability table. Fixed by splitting the body into markdown sections,
detecting deferred-intent headings via a keyword regex, and skipping
those sections when collecting IDs to check against the table.

Both fixes include boundary tests (false positive suppressed; genuine
gap/missing warnings still fire) in 4-phase-complete-cjs-regression.test.cjs.

Wire the already-reserved `phase.uat-passed` alias (subcommand `uat-passed`,
mutation:false) into the phase command router with a new markdown-aware
predicate that evaluates HUMAN-UAT results and reports pass only when every
required check passes. Post-SDK-retirement (ADR-0174/#174) successor to the
SDK-framed #70, with no SDK-specific API surface.

New pure module src/uat-predicate.cts:
- stripFalsePositiveContexts: frontmatter -> HTML-comment -> CommonMark-style
  fenced-block state machine (tracks delimiter char+length) -> blockquote,
  each a small composable step, so a `result: passed` inside frontmatter, a
  fenced/~~~ block (incl. ~~~ nested in a ``` fence), a comment, or a
  blockquote is never counted.
- parseUatResultItems: heading-block parser, column-0-anchored same-line
  result; a heading with no result -> `missing` (fail-closed).
- analyzeMarkdown: unterminated fence/comment detection (malformed -> blocker).
- evaluateUatPassed: allowlist pass/verification semantics; passed = no
  blockers && >=1 check && all passing; no_uat_artifacts discriminator (no
  vacuous pass); optional requireVerification policy hook.

Thin cmdPhaseUatPassed handler in phase.cts; router closure rejects unknown
flags via makeInvalidArgs. Hardened across two Codex adversarial passes
(vacuous pass, dropped failing tests, permissive verification status,
nested-fence escape, cross-line result value, masked unterminated comment) —
all fixed fail-closed. New unit + CLI-integration suites incl. a fast-check
property test; docs, CONTEXT glossary, inventory, and changeset updated.
