* chore: rebuild ADR index as a generated artifact and enforce lifecycle invariants
The ADR index in docs/adr/README.md was hand-maintained with nothing checking
it, and had drifted to 40 of 65 ADRs. The absent rows included the entire
capability family (857/894/959/1016/1143/1213/1244) and ADR-1239 (EoS) itself,
so the decisions a reader most needed were the ones they could not find.
Make the index a derived artifact, matching the repo's existing generated-file
idiom (lint:generated-sync), and enforce the corpus' lifecycle invariants:
- scripts/gen-adr-index.cjs generates the index between markers and validates
the status vocabulary (Accepted/Proposed/Superseded/Legacy/Retired),
successor links, id/filename agreement, and supersession symmetry.
- Wire --check into lint:generated-sync so drift fails CI.
Correct the lifecycle metadata the gate surfaced, without flipping any status:
- ADR-1239 (EoS) declared it subsumed ADR-1016/58/3660/894; none recorded it.
Add reciprocal "Subsumed by" pointers + dated amendments. Subsumption keeps
the target Accepted -- these are live adapters, not dead decisions.
- ADR-857/894 carry dated status caveats: they read Proposed while the
capability system shipped and epic #857 is closed. Ratification is a
maintainer act and is deliberately left open.
- Link ADR-0005/0007/0012/3524 -> ADR-0174 and ADR-0010 -> ADR-0009; record
the reciprocal Supersedes on ADR-0009.
- ADR-218 declared itself "ADR-0175" -- an unfinished rename.
- The 0011 PRD moves from the non-canonical "Draft" to "Legacy".

* test: capture stderr via spawnSync; record ADR-0010 draft supersession
Two fixes surfaced by the first gsd-test run and by regenerating the index:
- tests/adr-index-gate.test.cjs used execFileSync, which only surfaces stderr
through the thrown error on non-zero exit. The `--write` path exits 0 while
reporting outstanding violations on stderr, so the helper always saw ''.
spawnSync captures both streams on both outcomes.
- The hand-maintained index recorded 0010-skill-surface-budget-module.md as
"earlier draft superseded by ADR-0011" while the file itself still said
Proposed. Deriving the index from the files would have dropped that
assertion and resurrected a superseded draft as a live decision, so it is
recorded at its source, with the reciprocal Supersedes on ADR-0011.

* fix: drop the dead sdk/ model-catalog candidate retired by ADR-0174
src/model-catalog.cts resolved model-catalog.json through three candidates, the
second being sdk/shared/model-catalog.json three levels up. That was the legacy
source-repo fallback kept by the #3288 fix ("check the co-located path FIRST,
before the legacy source-repo path").
ADR-0174 then retired the @opengsd/gsd-sdk package boundary and deleted the sdk/
tree (11918dcc3), so the candidate can no longer resolve in any layout: a source
repo has no sdk/, and an install layout points it at ~/.claude/sdk/shared/, which
the installer never writes -- the original #3288 bug. It was dead weight implying
a package boundary this repo no longer has.
No test depends on it: the #3288 regression tests in tests/install.test.cjs write
their own synthetic old-path fixture and assert it throws.

* docs: ratify nine shipped ADRs; record why ten others stay Proposed
The corpus carried 19 Proposed ADRs, most describing architecture that had
already shipped. A Proposed label on live architecture tells contributors and
agents the decision is an unbuilt idea -- the capability system and EoS were
both being misread that way.
Audited all 19 against the shipped tree and GitHub. Each candidate flip then had
to survive two independent reviewers instructed to refute it.
Ratified Proposed -> Accepted, each with a dated Ratification section carrying
the verified evidence (file:line, symbols, tests, issue state):
857 capability system 894 declaration format 1244 capability ecosystem
1577 injection boundary 1610 size-budget ratchet 1990 existing-code onboarding
15 cross-AI convergence 22 plan-drift guard 0011 default reviewers
Held ten, each now carrying a "Why this is still Proposed" section naming the
blocker and its unblock condition, so the audit is not repeated:
2264 its own headline acceptance criterion is unmet in the tree
230 live branch protection contradicts the decided spec (1 approval, not 2)
660 the namesake release/<version> re-cut is manual, not automated
959 issue #2346 is approved and plans its graduation as its own ADR
1213 the shipped writer's return shape differs from the decided interface
443 the orchestrator override path has no live caller
1143 / 1606 each states its own bar for acceptance; neither is met
612 / 1671 legitimately open
Shipped code proved necessary but not sufficient: eight ADRs had every named
module, symbol, and test present with their epics closed, and still failed the
bar. That lesson is written into README.md's ratification procedure.
Also corrected ADR-857's "Supersedes (generalizes)" to "Subsumes": taken
literally it would have marked two live seams dead -- ADR-0011 (surface.cts:348)
and ADR-58 (runtime-artifact-install-plan.cts:82). Both keep Accepted status and
gain Subsumed-by pointers.
Index: Active 39->48, Proposed 19->10, Superseded/Legacy 7. 65 total.

* fix: harden gen-adr-index against hostile titles and non-ADR filenames (#2356)
Three findings from the pre-PR orthogonal security review, all confirmed:
- An ADR title containing the literal ADR-INDEX:END marker was emitted verbatim
into its table cell, relocating the splice boundary so the NEXT --write
spliced against the wrong marker and truncated README.md. Titles now render
through cellText(), which escapes pipes and angle brackets -- making an HTML
comment (and any other HTML) unformable from ADR-authored text.
- A docs/adr/*.md without a numeric prefix crashed on match(...)[1] of null.
Such a file is also invisible to the index -- the very failure this gate
exists to prevent -- so it is now reported as a naming-convention violation
naming the file and the fix.
- Tests leaked their mkdtemp dirs. They now use helpers.createTempDir/cleanup
via t.after(); helpers.cleanup carries the Windows-EBUSY retry budget that a
raw fs.rmSync lacks (caught by local/no-raw-rmsync-in-tests).
Adds five regression tests: marker hijack, HTML injection, pipe cell-break,
non-conforming filename, and splice stability across repeated writes.

* fix: close two gate false-passes; read ## Supersedes sections (#2356)
Second round of confirmed findings from the pre-PR orthogonal code review. Both
false-passes matter more than a false-fail: a gate that silently misses a
violation is worse than no gate, because it is trusted.
- A relation field mixing a link with a bare id silently dropped the bare claim:
the check tested `rel.links.length` (does this field have ANY link?) instead
of whether THAT id was linked. `Supersedes: [ADR-0001](...), ADR-0011` passed
clean -- accepting exactly the ambiguous bare reference the rule forbids. Now
each bare id is checked against the ids actually linked in the same field, so
a repeat in trailing prose stays quiet while an unlinked claim is flagged.
- The ratification guard (`statusToken !== 'Accepted'`) skipped BOTH relation
directions, which killed the IN check entirely: `supersedes.in` is only ever
populated on an ADR whose status IS `Superseded`, so a dangling `Superseded by
X` where X never claims it always passed. The guard now applies to OUT only --
a prospective claim must not obligate its target, but an ADR's statement about
ITSELF is always owed a reciprocal.
- Fixing that surfaced a parser gap: ADR-0174 declares its supersessions in a
`## Supersedes` table SECTION, not a header field, and headerBlock() stops at
the first `##`. The repo's best-documented supersession was invisible. Section
form is now parsed for both relations.
- Replaced a vacuous test: the em-dash negation case passed whether or not
NEGATED_RELATION_RE matched (a mutation to /$^/ survived). It now carries a
link that would create a failing asymmetric relation if negation did not fire.
Also removes docs/adr/9401-test-target.md -- a synthetic fixture a reviewer
created in the worktree while reproducing a finding, swept in by `git add -A`.
Adds regression tests for each: mixed link+bare, linked-and-repeated-in-prose,
dangling superseded-by from a non-Accepted ADR, and the ADR-0174 section shape.

* fix: escape backslashes before pipes in the ADR index cell renderer (#2356)
CodeQL js/incomplete-sanitization (high) on scripts/gen-adr-index.cjs: cellText()
escaped `|` -> `\|` without first escaping the backslash. Markdown's escape
character is the backslash, so the input `\|` became `\\|`, which renders as a
literal backslash followed by an UNESCAPED pipe -- re-opening the cell break the
pipe escape exists to prevent. Order is load-bearing: escape the escape
character first, then everything that emits one.
Same class as the index-marker hijack fixed earlier: ADR-authored text breaking
out of the cell it is rendered into.
Adds a regression test asserting a `\|`-bearing title leaves exactly the row's
own 5 unescaped delimiters and cannot forge a Status cell. Uses split(/\r?\n/)
per local/no-crlf-fragile-split -- a literal "\n" split is CRLF-fragile on the
Windows CI leg.

Records the decision (ADR-2346) to dissolve runCommand's 73-case switch into a
two-layer dispatch (registry families + leaf-verb table filling the prepared
_dispatchNonFamily seam), collapsing it to ~15 lines. Covers the four decisions
ADR-959 leaves open: full dissolution, family/leaf classification rule, shared

PR-0 of the #612 tracer-bullet sequence: the ADR that locks the contract
PR-1..PR-6 execute against. No production code.
Rewrites the design for current next (v1.7.0-rc.5): phase-id surfaces now
live in src/phase-id.cts under the ADR-2121 single-owner regime (core.cts
retired, #1267), and M-NN is a shipped first-class convention rather than a
no-adopter RC intermediate. States every blocking requirement from the
approved-enhancement comment as an explicit design commitment:
- terminal M-NN deprecation, end state two conventions (null + bracket) by
forward consolidation via the migrator, not "no adopters"
- EMIT/RENDER as one pure pair with fast-check round-trip properties, inside
phase-id.cts under the #2128 token-source / drift-lint regime
- single-sourced, generated PR-6 injection block + verify parity check
- migrator dry-run-default / dirty-tree guard / atomic rollback (the current
base's surgical reverse-rename #1542, a deliberate improvement over the
requirement's literal "HEAD-sha reset" — flagged) + two-invariant fixture
corpus + M-NN lift + HARD-REFUSE on absent project_code
- everything gated on phase_id_convention === 'bracket'; null / M-NN paths
byte-untouched (never gate on project_code)
- concrete collision anchor normalizePhaseName('2-01.02-01') === '02',
grounded in the live regexes at src/phase-id.cts:71/79
Guardrail: bracket is core config-gated behavior, not an add-only capability.

Phase 0 of the #2121 epic — an ADR-only PR that LOCKS the contract Phases
1-4 execute against. No production code lands here.
Locks:
- phase-id.cts as the single canonical owner of phase-identifier parsing.
- New pure exports Phase 1 adds: parsePhaseFromProse (anchored; fixes the
#2111 "Milestone v0.5 complete -> 5" class), stripConfiguredProjectCodePrefix
/ isForeignPrefixedPhaseQuery (config-aware; the #2104 fix's home),
and roadmapPhaseLookupSources moved in as sole owner of the 3-source
ordering (fixes the #2114 2-vs-3-source divergence).
- Extend-never-mutate on the 12 existing exports (normalizePhaseName has a
CRITICAL 84-symbol / 20-caller blast radius) — Hyrum's Law.
- The exact exact->numeric->prefix-tolerant lookup ordering.
- A behavioral anti-divergence contract: reference-identity guard +
scripts/lint-phase-id-drift.cjs scanner, modeled on the repo's proven
capability-precedence-parity / package-identity-drift patterns.
#2104 remains blocked on PR #2105 and off this epic's critical path.

* chore(context): scrub nul byte from consent-store predicate
CONTEXT.md line 206 (Capability Consent Store predicate) contained a
literal NUL byte between ${realpath(projectRoot)} and <id> documenting
the disk-key join format. The byte was intentional but made the file
binary-detected, breaking grep/rg searches (hit while preparing ADR-1817).
Replace with the 4-char \x00 escape. Preserves the byte-level disk-key
documentation; surrounding prose 'prototype-pollution-safe NUL-joined
keys' carries the semantic context. file(1) now reports 'Unicode text'.
* docs(#1817): add adr-1817 state.md rebuild derivability contract
Phase 0 of approved feature #1817 (epic). Lands the design contract for
the new `rebuild` transition in the STATE.md Transition Module (ADR-1769):
- ADR-1817 (new): `rebuild` is the capstone 11th transition. Six design
decisions: (1) core substrate, non-toggleable, same tier as the other
10; (2) section taxonomy — re-derivable (`## Current Position` prose
from frontmatter, `## By-Phase Progress` table from disk) vs preserved
(`## Session`, `## Decisions`, unknown sections) vs de-duplicated
(`## Session Continuity Archive`); (3) orphaned data is logged + dropped
with a structured audit entry in `## Rebuild Log` (ADR-1411 provenance
principle); (4) idempotency is a hard guarantee — a no-mutation rebuild
appends no log entry; (5) non-overlapping scope with `sync` (3
frontmatter fields, auto-triggered); (6) orthogonal to
`auto_prune_state` (rebuild reconciles with current canonical sources,
prune removes by retention policy).
- CONTEXT.md (STATE.md Transition Module section): list `rebuild` as the
11th intent; add contract predicates mirroring the ADR.
- docs/adr/README.md: add ADR-1817 to the index (Accepted).
Targets the #1776/#1761/#1591 body-drift cluster that survived ADR-1769's
per-field transitions. Phased per ADR-1817: this PR (Phase 0) closes
#1817; Phase 1 (#1827) lands `rebuildCore` + intent dispatch + drift-class
unit tests; Phase 2 (#1826) lands `cmdStateRebuild` CLI + dry-run +
integration tests + docs + changeset.
* docs(#1817): reword state-doctor alternative to satisfy docs-parity lint
The docs-parity-live-registry test scans every docs/*.md (including
docs/adr/) for slash-command tokens and asserts each one resolves to a
live command in the registry. The rejected-alternative #4 in ADR-1817
mentioned a hypothetical `/gsd:state-doctor` workflow, which tripped
the lint (`unknown command token(s): [/gsd:state-doctor]`).
Reword to 'standalone state-doctor workflow' (no slash prefix). The
extractor is aggressive — backticks and space-preceding tokens are both
extracted per the test's own polarity-invariant cases — so the only
sound fix is to not form a slash token at all for hypothetical names.
Verified locally: `node --test tests/docs-parity-live-registry.test.cjs`
now passes 31/31 (was 30/1).
Phase A of epic #1258. Adds the single authoritative ADR documenting
the per-runtime skill mapping + converter transform-contract catalog

Records the design gate for moving agent conversion off the inline bin/install.js loop onto the descriptor path (ADR-3660): the two-(really three-)path problem, the 10 parity behaviors the descriptor agents path must gain (verified against code — the issue's 7 plus Qwen/Hermes branding, the Codex TOML sidecar, and stale-agent cleanup), an AgentConverterContext contract to fix the leaky (content)=>string converter signature, and an incremental per-runtime cutover gated on byte-for-byte golden parity (full + minimal mode). Status: Proposed. Codex-reviewed for technical accuracy.

Realize ADR-857 Branch 8 (host-CLI support as role:runtime Capabilities) +
materialize ADR-58's InstallPlan. Closes the 6-axis descriptor vocabulary,
absorbs the hard-case runtimes as data, and stages the install migration as a
5a→5g ladder (4/6 axes already modularized; InstallPlan is the 5g capstone,
reachable by collection, not a big-bang rewrite).
Amended after a plugin-side design grill (rubber-duck + grill-with-docs vs
#956 MemPalace / #999 Impeccable):
- New term Connected Capability (CONTEXT.md) — a Capability whose integration
shape brings its own external process/service/state; orthogonal to authorship.
Named, tracked gap (vehicle #956); current schema does not express it.
- Narrowed the dogfood claim: the descriptor dogfoods the runtime interface
only, not the feature-plugin/Connected path.
- Structural "off means off" rule (CONTEXT.md RULESET.CAPABILITY.off-means-off):
the host derives shared outputs from active hooks; a hook adds/is-counted,
never mutates host source. Ratify in ADR-894; proven by spike #1018.
- Hand-waves resolved by code: sandboxTier real-but-thin; model-catalog
orthogonal (not a 7th axis); converters closed into a ConverterName enum +
added the kimi-agents artifact kind; configHome is pure read-only.
- Hook-firing path is unproven (render-hooks built, never consumed) → spike
#1018 must prove render-hooks→live-workflow execution before phase-5 build.
Design-only; no code. Status: Proposed.

boundary: install policy projects a pure, typed install plan by

* feat(routing): add CommandRoutingHub with behavioral test suite (#3788)
Introduces createHub({ mode, sdkLoader, cjsRegistry, manifest }) and
hub.dispatch({ family, subcommand, args, cwd, raw }) -> Result with a
closed 6-value ERROR_KINDS frozen enum. Hub never throws, never prints,
and enforces no transparent fallback between sdk/cjs modes. 34 behavioral
tests cover all errorKind values, mode fixation, and the no-throw contract.

* refactor(routing): migrate phase-command-router to CommandRoutingHub (#3788)
Rewrites phase-command-router.cjs to dispatch through CommandRoutingHub.
Public entry point routePhaseCommand({ phase, args, cwd, raw, error }) is
unchanged. The adapter determines mode (sdk/cjs) from env + tryLoadSdk(),
constructs a hub, dispatches, and translates the pure Result back to
output()/error() calls. New behavioral test suite (23 tests) replaces the
old mock-heavy approach and includes two integration tests through the real hub.

* fix(routing): preserve phase.mvp-mode JSON error and ROADMAP scan through hub (#3788)
mvp-mode was never registered in the SDK; the pre-#3788 CJS router
always dispatched it via the CJS handler even when sdkAvailable was
true. After the hub migration, SDK-mode hubs (Docker, where the SDK
build exists) sent mvp-mode to the SDK bridge, which returned
SdkDispatchFailed with reason 'unknown' instead of the expected
'usage' code, and failed ROADMAP lookups. Fix by short-circuiting
mvp-mode to the CJS handler before hub construction, matching the
pre-migration observable behaviour.

* docs(adr): note SDK-incomplete subcommand limitation in ADR-0012 (#3788)
* fix(inventory): bump CLI Modules headline to 74 after rebase onto main (#3788)
Upstream added code-review-flags.cjs (72→73) at the same time our branch
added command-routing-hub.cjs. After rebase both modules exist (74 total)
but the headline stayed at 73; bump to 74.
* fix(routing): remove dead mvp-mode handler from cjsRegistry (#3788)
The cjsRegistry['phase']['mvp-mode'] handler (previously lines 65–68)
was unreachable: the early-return bypass at line 56 intercepts mvp-mode
before hub construction in CJS mode, and in SDK mode cjsRegistry is
passed as undefined. Remove the dead handler; all 57 tests still pass.
* docs(adr): correct router count in ADR-0012 (#3788)
The context section cited "eight" routers including "frontmatter" but
there is no frontmatter-command-router.cjs. The actual count is seven:
phase, phases, roadmap, state, verify, validate, init.
* fix(routing): guard missing subcommand + use ERROR_KINDS constant (#3788)
Two fixes in phase-command-router.cjs:
1. Add early-return for missing subcommand before hub construction.
Pre-#3788 the routeCjsCommandFamily fell through to error() for
undefined args[1]; post-#3788 the hub's manifest check skips falsy
subcommands, which would have sent bare 'phase' into SDK dispatch
in SDK mode instead of the expected "Available: ..." error message.
2. Switch on ERROR_KINDS.UnknownCommand instead of bare 'UnknownCommand'
string, per ADR-0012's closed-enum contract ("callers switch on
ERROR_KINDS values, not bare string literals").
* docs(routing): fix factual errors in ARCHITECTURE, ADR-0012, changeset (#3788)
Three corrections:
1. ARCHITECTURE.md: softened "All CJS command family routers dispatch
through CommandRoutingHub" — only phase-command-router.cjs is
migrated in this PR; remaining routers still use routeCjsCommandFamily
and migrate in follow-up issues.
2. ADR-0012: corrected the SDK mvp-mode claim. The ADR said "the SDK
has no equivalent entry" but sdk/src/query/command-static-catalog-
domain.ts:104-105 registers phase.mvp-mode. The actual reason for
the early-return bypass is divergent ROADMAP scan behaviour and
error reason codes, not SDK absence.
3. .changeset/mellow-tigers-gather.md: corrected pr: 1 → pr: 3828.

* docs(3660): add Runtime Artifact Layout Module ADR + CONTEXT.md entry
Records the architectural decision to add a Runtime Artifact Layout Module
that owns per-runtime artifact placement (commands / agents / skills) as a
typed seam. Closes the bug class behind #3659 where the Runtime Surface
Module forgets artifact kinds the install/uninstall pipelines track.
CONTEXT.md gains the new module entry directly after the Skill Surface
Budget Module since the two seams compose.
Pure docs — no code changes. Phase 1 implementation lands in a separate PR.

* docs(3524): propose CJS↔SDK hard-seam ADR + phased PRD
Adds docs/adr/3524-cjs-sdk-hard-seam.md (Proposed) and
docs/prd/3524-cjs-sdk-hard-seam.md (Reference) tracking #3524.
Updates the ADR and PRD index READMEs.
The ADR defines one canonical owner per responsibility across the
CJS (bin/lib/*.cjs) and SDK (sdk/src/**/*.ts) sides, eliminating the
recurring drift bug class (#1535, #1542, #2047, #2638, #2653, #2687,
#2798, #3055, #3523). Three layers: shared data (sdk/shared/*.json),
shared core logic (sdk/src/core/ → dual CJS+ESM build), thin adapters.
Enforcement is layered: build-time grep, type-level contract test,
mutation parity test, CODEOWNERS gate, in-file banner.
The PRD phases the migration in five independently shippable steps —
shared data first (closes constant drift), then config consolidation
(closes#3523 class), then project-root + path projection, then state
and verify handlers, then enforcement hardening + retrospective.
* docs(3524): revise seam ADR + PRD after architecture review
Architecture-review pass (via /improve-codebase-architecture) found
seven deepening opportunities; this commit applies all of them.
1. Re-anchor on the existing generator precedent. The repo already
has sdk/scripts/gen-command-aliases.ts emitting both
.generated.ts and .generated.cjs from one TS source, with
sdk/scripts/check-command-aliases-fresh.mjs as the CI freshness
gate. The ADR's invented dual CJS+ESM bundler pipeline is
dropped. Each Shared Module gets one generator script and one
freshness check, modeled on that precedent.
2. Drop the generic sdk/src/core/ container. The canonical-owner
table is now indexed by Module, using the CONTEXT.md domain
vocabulary (STATE.md Document Module, Configuration Module,
etc.) rather than file-path-based pseudo-modules.
3. Split the coarse "State management" row into three: the pure
STATE.md Document Module (already a character-identical
hand-synced pair — perfect Phase 1 target), the Planning
Workspace Module (defer to ADR-0004), and per-side state I/O
Adapters (legitimately differ sync vs async).
4. Defer to existing ADRs. Planning Path Projection (ADR-0006),
Model Catalog (ADR-0003), Planning Workspace (ADR-0004),
Dispatch Policy (ADR-0001), Shell Command Projection (ADR-0009
post-Phase 3-4 expansion which absorbed superseded ADR-0010).
The stale ADR-0010 reference is fixed.
5. Define a Configuration Module entry in CONTEXT.md as a Phase 2
deliverable, with explicit Interface contract for loadConfig,
normalizeLegacyKeys, mergeDefaults, migrateOnDisk.
6. Split the Workstream Inventory Module into a pure Builder
(generated, shared) and per-side Reader Adapters (hand-authored,
sync vs async). Same pattern generalizes to other paired Modules.
7. Match enforcement to existing scripts. Per-Module freshness
checks (precedent: check-command-aliases-fresh.mjs), per-Module
drift lints (precedent: lint-shell-command-projection-drift.cjs),
and one hand-sync pair lint that blocks the #3523 anti-pattern
at PR time.
PRD phases reordered: STATE.md Document Module ships first as a
proof of pattern (two identical files become one source plus one
generated artifact). Configuration Module ships second, closing
the #3523 class. Workstream Inventory Builder split third.
Project-Root Resolution fourth. Enforcement and retrospective
fifth.
* docs(3524): expand scope — CJS router delegates to SDK runtime bridge
User flagged that the original "Out of scope" list was my unilateral
scoping call, not theirs. After review, the CJS router consolidation
(formerly out-of-scope item #1) is brought into scope.
ADR additions:
- CJS Command Router Adapter Module row added to canonical-owner
table. Existing Module (per CONTEXT.md) is amended so the
per-family `handlers` map delegates to `QueryRuntimeBridge.execute()`
in-process. Per-side CJS handler files for canonical families
(state.cjs, verify.cjs, init.cjs, phase.cjs, etc.) shrink to
delegates or are deleted.
- Per-side I/O Adapter consequence updated to clarify the bridge
preserves the in-process model. No subprocess hop is added.
- "Out of scope" stripped of router item; CJS-only seam migration
and verify-Module-first work remain out of scope.
PRD additions:
- New Phase 5: CJS Command Router Adapter delegates to SDK runtime
bridge, family-by-family, with golden parity matrix per family
gating each PR.
- Old Phase 5 (enforcement) renumbered to Phase 6, expanded to cover
Phase 5's parity matrix and runtime-bridge CODEOWNERS.
- Open question #4 added for the synchronous-bridging mechanism
(`deasync` vs `Atomics.wait` vs sync-handler refactor) — resolved
in the Phase 5 spike before any family migration begins.
- Open question #5 added for family migration order (recommended:
smallest read-only family first).
- Risks table expanded with three Phase 5 rows: bridging-mechanism
uncertainty, observable-output regression, startup-time impact.
- Done-when updated for six phases and five enforcement layers.
Non-goals updated: CJS-only Module migration and verify-Module
deepening remain out of scope. CJS CLI removal explicitly stays
off the table — the external `gsd-tools` contract is preserved.
* docs(3524): address CodeRabbit review
* docs(3524): fix PRD issue reference markdown
* docs: adopt issue#-prefix naming for ADRs/PRDs (#3485)
The repo's sequential ADR/PRD numbering convention has produced
recurring collisions when developers compute "next number" locally
and ship in parallel — currently visible on disk as duplicate
docs/adr/0010-*.md and triplicate docs/adr/0011-*.md, plus a stack
of "resolve ADR conflict" commits in git history.
Replace the local-compute convention with issue#-prefix slug naming:
docs/adr/<issue#>-<slug>.md (new ADRs)
docs/prd/<issue#>-<slug>.md (new PRDs — directory introduced)
GitHub issue numbers are server-assigned and atomic, so the
reservation step the CONTRIBUTING.md issue-first rule already enforces
also produces the artifact ID. One issue = one ADR-or-PRD = one PR.
Same shape as the existing changeset random-name pattern (#2975) for
CHANGELOG.md fragments, applied to a different artifact class.
Migration policy: legacy ADRs 0001-* through 0011-* are preserved
as immutable historical record. The new convention applies only to
ADRs/PRDs created on or after this merge.
Files updated:
- docs/adr/README.md — naming convention + legacy note + link
- docs/prd/README.md (new) — seeds the new directory + same convention
- CONTRIBUTING.md — new "Proposing an ADR or PRD" section
- docs/contributor-standards.md — formalize as contributor requirement
No code surface — docs-only.

* docs(changeset): add Changed fragment for ADR/PRD naming convention (#3487)
Per CONTRIBUTING.md "When unsure whether a change is user-facing, add
the fragment" — the contributor process IS user-facing for the
contributor user class. Drop the no-changelog opt-out, surface the
naming-convention change in the next CHANGELOG so contributors see
it before they hit it as a PR rejection.

* docs: address CodeRabbit findings on #3487
- CONTRIBUTING.md: rename heading to "Proposing an ADR or PRD" so its
GitHub-anchor slug matches the #proposing-an-adr-or-prd link target
used from docs/adr/README.md, docs/prd/README.md, and
docs/contributor-standards.md (broken anchors)
- docs/adr/README.md, docs/prd/README.md, docs/contributor-standards.md:
add `text` language tag to the new naming-convention fenced blocks
to satisfy markdownlint MD040
Pre-existing untyped fences elsewhere in the touched files are left
alone per CONTRIBUTING.md "no drive-by formatting".

* feat(skill-deps): add requires: frontmatter to all 51 skills with cross-skill references
Mechanical migration from docs/research/data/2026-05-12-skill-audit.json.
Every skill whose body references another GSD skill now declares those
dependencies in `requires:` YAML frontmatter (flow-style array).
Notable: discuss-phase, plan-phase, and execute-phase all reference `phase`,
which confirms the latent gap in MINIMAL_SKILL_ALLOWLIST — `phase` is pulled
by the core loop but was never in the allowlist. The profile closure model
(ADR-0010 Phase 1) resolves this automatically.
Closes part of #3408.

* feat(skill-surface-budget): add PROFILES map, resolveProfile, loadSkillsManifest, staging, marker IO
Implements the Skill Surface Budget Module core (ADR-0010, Phase 1):
- PROFILES Object.freeze map: core (6 skills), standard (~13), full ('*')
- loadSkillsManifest: parses requires: frontmatter from commands/gsd/*.md
into a Map<stem, string[]> without external YAML dep
- resolveProfile({modes, manifest}): computes transitive closure over the
requires: graph; composable (modes=['core','audit'] unions closures)
- stageSkillsForProfile / stageAgentsForProfile: filesystem staging with
same exit-cleanup machinery as the legacy stageSkillsForMode
- readActiveProfile / writeActiveProfile: .gsd-profile marker round-trip
- Back-compat shims preserved: MINIMAL_SKILL_ALLOWLIST, isMinimalMode,
shouldInstallSkill (overloaded), stageSkillsForMode — all legacy tests pass
The phase latent bug is now resolved by closure: discuss-phase, plan-phase,
and execute-phase all require phase, so any profile including any of them
automatically includes phase via transitive closure.
Tests: 22 manifest+resolve, 9 stage, 10 marker (41 new tests, all green).
Back-compat anchor: 80/80 passing.
Closes part of #3408.

* feat(skill-surface-budget): add lint-skill-deps.cjs CI gate and fix 19 missed requires: entries
Two lint checks (scripts/lint-skill-deps.cjs):
a) Frontmatter-body consistency: skill body references must appear in requires:
b) Profile closure: every requires: dep of any profile skill must be in closure
Running the lint revealed 19 body references missed by the audit JSON (the
audit used static analysis; some bodies have conditional references). Fixed:
complete-milestone: +audit-milestone, discuss-phase, plan-phase, execute-phase, new-milestone
fast: +quick
health: +thread
map-codebase: +new-project, plan-phase
new-milestone, new-project, review, ultraplan-phase: +plan-phase
ship: +verify-work
sketch, spike: +new-project
verify-work: +execute-phase
workstreams: +new-milestone, resume-work
Wired into package.json as lint:skill-deps and added to pretest.
8 fixture-based tests: all green.
Closes part of #3408.

* feat(skill-surface-budget): wire --profile= arg, profile marker write/read in bin/install.js
- Add --profile=<name> / --profile=<n1>,<n2> arg parsing (composable).
Mutually exclusive with --minimal / --core-only (aliases for --profile=core).
Default (no flag): full.
- Import readActiveProfile / writeActiveProfile from install-profiles.cjs.
- After writeManifest: persist active profile to .gsd-profile marker.
- gsd update path: if no --profile flag given, read existing .gsd-profile
marker so non-full profiles are not silently re-expanded to full (ADR-0010).
- Update --help block to document --profile= with per-tier token costs.
New test: install-minimal-backcompat.test.cjs (6 tests):
- PROFILES.core === MINIMAL_SKILL_ALLOWLIST (contract)
- --minimal writes .gsd-profile marker "core"
- --profile=core, --profile=standard write correct markers
- default install writes marker "full"
Closes part of #3408.

* chore(changeset): add feat-3408-skill-profiles changelog fragment
Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
* feat(install-profiles): derive agents from skill body refs and wire into resolveProfile
Deviation 1 of ADR-0010 phase 1b: tiered profiles (core, standard) now produce
a non-empty agents Set instead of always returning empty. resolveProfile() scans
each skill body for gsd-* agent name references (via new parseCallsAgents()),
stores them in _calls_agents_<stem> manifest entries, and unions them across the
resolved skill closure. stageAgentsForProfile() already checked resolvedProfile.agents
— it now gets real data so tiered profiles install the correct subset of agents
instead of zero.
Closes#3408 (partial — Deviation 1 only)
Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
* feat(install): honor .gsd-profile marker on update, add resolveEffectiveProfile/mostRestrictiveProfile
Deviation 2 of ADR-0010 phase 1b: the marker written during installation is now
actually honored when re-running without explicit flags (e.g. gsd update). The
dead-end logging block is replaced by resolveEffectiveProfile(), which picks the
marker profile over 'full' when no explicit --profile= flag was given. The resolved
profile is piped through to all 13 stageSkillsForMode dispatch sites (now _stageSkills)
so updates install only the previously-chosen skill subset.
--minimal retains its back-compat behavior (strict 6-skill allowlist, no closure)
while writing 'core' to the marker. mostRestrictiveProfile() is exported for callers
that need to reconcile disagreeing markers across runtimes (smallest skill set wins).

* feat(surface): add CLUSTERS data + state IO module
Add clusters.cjs with 10 named skill groups covering all 66 skills
(verified by surface-clusters.test.cjs). Add surface.cjs with readSurface/
writeSurface atomic IO, resolveSurface, applySurface, and listSurface.
Tests: 17 passing (11 state IO + 6 cluster integrity).
Closes#3408
* docs(adr): add ADR-0011 Skill Surface Budget Module (Phase 1 accepted, Phase 2 amendment)
Records the install-time profile staging decision (Phase 1, landed) and the
runtime /gsd:surface cluster-toggle decision (Phase 2, in flight) as an
amendment. Updates the ADR README index.
Closes#3408
* docs(install-profiles): update module docblock for Phase 2 and ADR-0011
Corrects the ADR reference from 0010 to 0011, documents the three-profile
model and back-compat aliases, adds resolveEffectiveProfile precedence rule,
and notes the companion surface.cjs Phase 2 engine.
* docs(context): add Skill Surface Budget Module canonical entry
Adds the Domain terms entry for the Skill Surface Budget Module covering
both Phase 1 (install-time profiles, .gsd-profile marker) and Phase 2
(runtime /gsd:surface cluster toggles, clusters.cjs, .gsd-surface.json),
per ADR-0011 Consequences requirement.
* feat(surface): add resolveSurface and applySurface engine + tests
Tests cover: profile → surface equivalence, cluster disable/enable,
explicitAdds transitive closure, applySurface file sync (add missing,
remove superseded, preserve non-gsd files), listSurface token cost.
16 new tests passing.
* docs(readme): document --profile= flag and /gsd:surface command
Brief user-facing mention of install profiles (core/standard/full) and the
/gsd:surface slash command in the Commands table. Points to ADR-0011 for details.
* feat(surface): add /gsd:surface slash command runbook
New skill: gsd:surface — runtime profile/cluster toggle without reinstall.
Sub-commands: list, status, profile <name>, disable/enable <cluster>, reset.
Persists state to .gsd-surface.json (independent of .gsd-profile).
Description 96 chars (≤100 limit). lint:descriptions + lint:skill-deps: 0 violations.
* feat(surface): add changeset fragment for /gsd:surface runtime toggle
* feat(surface): add surface skill stem to utility cluster
surface.md is a new skill; add it to the utility cluster so the
surface-clusters.test.cjs coverage invariant stays satisfied.
* docs(adr): fix ADR references to 0011 and record Phase 2 as shipped
ADR-0010 number was already claimed by the file-operation-engine ADR; this
ADR landed as 0011-skill-surface-budget-module.md. Update inline ADR
references in clusters.cjs, surface.cjs, install-profiles.cjs, and the
Phase 2 changeset to ADR-0011. Update the ADR Status section to record
Phase 2 artifacts as shipped on this branch rather than "in progress".
Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
* docs(research): port skill-surface-budget memo and audit data
ADR-0011 references docs/research/2026-05-12-skill-surface-budget.md and
docs/research/data/2026-05-12-skill-audit.json, which only existed in the
research worktree. Port both onto this branch so the ADR's References
section resolves and reviewers can read the cluster taxonomy (§3.2),
dependency topology (§3.1), and option grading (§4) that justify Phase 1
and Phase 2 decisions.

* fix(registration): register surface/clusters in INVENTORY, COMMANDS, and help.md
- surface.md: convert allowed-tools from inline YAML array to block style
(was parsed as a single tool name "[Read, Write, Bash]" by test harness)
- docs/INVENTORY.md: add CLI module rows for clusters.cjs and surface.cjs;
add Commands row for /gsd-surface; bump CLI Modules count 55→57, Commands 66→67
- docs/INVENTORY-MANIFEST.json: add entries for clusters.cjs, surface.cjs,
and /gsd-surface (filename-based command key)
- docs/COMMANDS.md: add ### `/gsd-surface` heading in Configuration Commands
- get-shit-done/workflows/help.md: add /gsd:surface entry in Configuration section
Fixes registration failures introduced by Phase 2 of #3408.

* fix(surface, docs): scrub .claude leakage and escape hypothetical slash tokens
Two PR regressions introduced earlier on this branch:
1. surface.cjs JSDoc comments contained the canonical paths
(~/.claude/commands/gsd, ~/.claude/agents) as example values, which the
cline-install leak regex (~\/\.claude\/(?:get-shit-done|commands|agents
|hooks)) flagged as install-time path leaks. Reworded the docblocks to
describe runtime-resolved paths without literal ~/.claude tokens.
2. The ported research memo proposed hypothetical Option C dispatchers
using slash syntax (/gsd:milestone, /gsd:research). The
docs-parity-live-registry test enforces that every slash-command token
in docs/ resolves to a real command. Rewrote the Option C sketch
without the slash prefix and added a clarifying note that the
dispatchers are illustrative, not shipped.
Targeted tests now pass: tests/cline-install.test.cjs and
tests/docs-parity-live-registry.test.cjs both green.

* docs(adr): add docs/adr/README.md index and structural ADR test (#3271)
- Add docs/adr/README.md as an indexed entry point linking all 7 ADRs
- Add tests/enh-3271-sdk-adr-structure.test.cjs: structural assertions that
ADR 0005 and 0006 exist, have required headings and Status/Date metadata,
and that README links every ADR file by filename
- Update CHANGELOG.md with Enhancement entry
- Add .changeset/3271-sdk-adr-structure.md
ADRs 0005 (SDK architecture seam-map) and 0006 (planning-path projection
module) already landed on main. This PR completes issue #3271 by adding the
README index and the structural test gate that enforces ADR completeness
going forward.
