9de4d67118 fix(#3579): a pointer-less session inherits the repo active-workstream marker (#3616)
* test(3579): failing-first coverage for repo-marker inheritance

A session that carries an identity but has never run 'workstream use' reads an
absent session pointer, resolves null, and composes the flat .planning tree even
when .planning/active-workstream names a live workstream. These tests fail on that
and pin the invariants the fix must not break: a session with its own pointer is
never repointed, and a session that merely lacked a pointer must never clear the
shared marker on another session's behalf.

* fix(3579): a pointer-less session inherits the repo active-workstream marker

RED proven at 157cae26: the three inheritance tests failed while every isolation and
negative control passed on base — the gap, and nothing else.

pickActiveWorkstreamAdapter returned exactly ONE adapter: the session-scoped one
whenever a session key existed, so the shared .planning/active-workstream marker was
never consulted. getWorkstreamSessionKey resolves a key from ~13 env vars or the
controlling TTY, so on any normal interactive terminal a key almost always exists —
which is why a session that had never run 'workstream use' read an absent pointer,
resolved null, and composed the FLAT planning tree even though the repo marker named a
live workstream. Reads misreported; writes corrupted the superseded flat STATE. Silent,
because the stale tree is well-formed.

This was a genuine design fork, not an oversight: references/workstream-flag.md
documented step 4 as a fallback 'when no session key exists', and the session isolation
that buys is deliberate (#2850). The issue's Agent Brief left the choice open and said
the reference doc should match whatever semantics ship. The maintainer ruled in chat for
inheritance.

Resolution now walks an ORDERED chain — session adapter first, shared second — and only
a null from the session adapter falls through to the marker. Strictly additive: it can
only turn a null into a name, never change a name that already resolves.

The dangerous part is clear() ownership. resolveFromChain treats chain[0] as owned: only
it is ever cleared, and only under selfHeal (getActiveWorkstream, never peek). An
INHERITED marker is read-only — a stale value there resolves null and the file is left
alone. Without that, one pointer-less session's read would delete the repo marker for
every other session, which is a worse bug than the one being fixed. Covered by a test
that asserts the marker still exists on disk after such a read.

peekActiveWorkstream inherits but still mutates nothing (#2850 — the statusline draws on
every render).

references/workstream-flag.md's Resolution Priority is rewritten to match, keeping the
session-isolation rationale and noting that inheritance does not weaken it: a session
that owns a pointer is never repointed.

Fixes #3579

* fix(3579): correct the guard diagnostics and lock the clear-semantics

Three review passes; every finding fixed inline.

MISSING ACCEPTANCE CRITERION (spec pass). The brief requires refusal diagnostics that
distinguish 'marker present but the session lookup missed it' from 'no workstream set at
all', and the two workstream-mode fail-safe guards were byte-for-byte untouched — still
emitting a generic 'no active workstream is set' even when a marker exists and merely
names a missing directory. Both guards (cmdPhaseComplete, cmdInitProgress) now branch on
a new read-only diagnoseUnresolvedActiveWorkstream, which reuses the SAME
resolvesToExistingWorkstream predicate resolveFromChain uses, so the diagnosis and the
resolution cannot disagree. Two typed reasons added to ERROR_REASON; both arms still
refuse — the fail-closed behavior is unchanged, only the message is now true.

REAL TEST FAILURE, not a flake. The remote run failed 'clearing one session does not
clear another session pointer'. That describe uses before() rather than beforeEach, so
one tmpDir is shared and an earlier test writes active-workstream=beta into it; under
inheritance the just-cleared session picks that marker up and resolves beta instead of
null. The failure is a CORRECT consequence of Option A surfaced through an
order-dependent fixture. The test now establishes its own marker state explicitly — its
real intent (clearing A must not disturb B's pointer) is preserved and not weakened — and
a new test pins the semantic deliberately: clearing a session pointer returns that
session to INHERITING the marker, it does not force flat mode. Documented in
references/workstream-flag.md, including how to actually get flat behavior.

Also from review: partial activeWorkstreamAdapters injection no longer silently
synthesizes a REAL filesystem adapter for the missing half (a latent test-isolation
trap); the duplicated validate-then-existsSync logic is factored into one predicate; and
the two try/finally test bodies are converted to t.after per CONTRIBUTING.

New coverage: whitespace/empty shared marker; a session whose OWN pointer is stale while
the marker names a different valid workstream (must self-heal to null, never inherit —
the isolation guarantee at its sharpest); and both new diagnostic arms asserted on
structured --json-errors output rather than prose.

* fix(3579): read resolvability with the non-mutating peek, not the self-healing resolver

Three of our own new tests failed on 7f5e706a. All three had ONE root cause, and none
was fixed by relaxing an assertion.

gsd-tools.cjs's bootstrap called the MUTATING getActiveWorkstream unconditionally on
every invocation, purely to populate routing env. On an unresolvable pointer that
self-healed — cleared it — BEFORE the dispatched command ran its own resolution. A second
read in the same process then observed already-cleared state:

- Isolation violation: a session whose own pointer was stale had it cleared by the
  bootstrap, so cmdWorkstreamGet's own resolution found a pointer-LESS session and
  inherited the shared marker ('beta' instead of null). Exactly the guarantee #2850 exists
  to protect, defeated across two calls rather than within one.
- Guard diagnostics: the guards' own truthiness check also used the mutating resolver, so
  it cleared the invalid marker and the immediately-following read-only diagnosis found
  nothing and reported none_active instead of marker_unresolved.

So a single invocation's answer depended on how many times it resolved. The bootstrap
self-heal is PRE-EXISTING and was harmless while pointer-less meant flat — inheritance is
what made it answer-changing, so this fix belongs here.

Every call site that only CHECKS resolvability — the bootstrap, both fail-safe guards'
truthiness check, and two informational init report fields — now uses the non-mutating
peekActiveWorkstream. Self-heal is unchanged in active-workstream-store and still fires
exactly once, at whichever site actually consumes the workstream.

Verified by driving the real CLI against temp fixtures, since the suite cannot run
locally: stale-own-pointer resolves null with the marker intact; both guard arms report
marker_unresolved with missing_workstream_dir / invalid_name and the marker survives;
no-marker still reports none_active; identity-less self-heal still deletes an invalid
marker byte-identically to pre-#3579; and a session with a valid own pointer still wins.

* chore(3579): backfill changeset PR number (#3616)

* test(3579): kill the surviving mutants in the new resolution code

CI's Stryker gate failed: active-workstream-store scored 79.45% against a break
threshold of 80 — 259 killed, 67 survived, at 'Ran 1.00 tests per mutant on average'.
The survivors cluster in the code this PR added (pickActiveWorkstreamAdapterChain,
resolvesToExistingWorkstream, resolveFromChain, diagnoseUnresolvedActiveWorkstream):
the CLI-level tests exercise those paths but do not DISCRIMINATE their branches, which
is precisely what a surviving mutant means.

Raised by strengthening assertions, never by touching the threshold. 21 unit tests added
to the existing unit suite, each written to fail under a specific named mutant, using the
module's injected adapter seams and createMemoryPointerAdapter so they stay hermetic
under Stryker's per-mutant reruns:

- chain shape with and without a session key, asserting length AND element identity
  (kills the if(false), the ': []' array mutant, and the block removal)
- partial adapter injection, asserting the missing half is an inert memory adapter that
  never touches the filesystem (kills the three '??' -> '&&' mutants)
- both arms of '!name || !validateWorkstreamName(name)' as SEPARATE tests — an absent
  name and a non-empty invalid one — which is what kills the '||' -> '&&' mutant
- self-heal discrimination: getActiveWorkstream must clear an unresolvable owned pointer
  and peekActiveWorkstream must not, asserted on adapter state after each
  (kills if(selfHeal) -> if(true))
- fallback arm both ways: a fallback that resolves and one that does not
- diagnoseUnresolvedActiveWorkstream asserted as a full object per case, with the reason
  strings compared exactly (kills present:true -> false and both StringLiteral mutants)

One mutant is deliberately left: 'if (chain.length === 0)' -> 'if (false)'. The branch is
structurally unreachable — the only chain source always returns a 1- or 2-element array
literal — and resolveFromChain is not exported. Killing it would mean exporting an
internal or deleting a defensive guard; neither is worth doing for a mutant, and the
score clears 80 without it. Recorded here rather than left unexplained.

Every new assertion was evaluated against the built module with real fixtures before
committing, since the suite cannot run locally.

0c43d853e2 fix(#1884): surface planning-lock mkdir failures, not a phantom timeout (#3472)
* test(#1884): reproduce phantom lock timeout from swallowed mkdir failure

withPlanningLock swallows a platformEnsureDir failure at src/planning-workspace.cts:210,
so an EACCES/ENOSPC/EROFS creating .planning/ is misreported as a 10s "held by a live
process" lock-contention timeout instead of the real filesystem error. These tests pin
the corrected contract and currently fail against the unfixed source (RED). Also proves
the pre-existing Docker overlay-fs lock-write retry race is unaffected by this change.

* fix(#1884): surface mkdir failures instead of a phantom lock timeout

withPlanningLock swallowed platformEnsureDir failures (EACCES/ENOSPC/EROFS/EMFILE)
creating .planning/, so the subsequent lock write failed with ENOENT (parent
missing), which is retryable (PLANNING_LOCK_RETRY_ERRNOS, added for a Docker
overlay-fs race). The loop then spun the full 10s budget and threw a phantom
"held by a live process" contention error pointing at a nonexistent holder.

The mkdir failure now propagates immediately with its real errno and message.
The Docker overlay-fs ENOENT lock-write race (directory present) is unaffected,
as is every path where .planning/ already exists or is creatable.

Per docs/adr/1411-resolution-provenance.md's 2026-07-26 amendment, this is the
one site in epic #1879 that legitimately throws (ADR-227's genuinely-fatal
carve-out) -- its defect was throwing the wrong error after swallowing the
real one, not that it threw at all.

2bead6ca1d feat(#3149): add dedicated init.debug entry point for /gsd:debug
/gsd:debug was one of the last workflows with no cmdInit* of its own: its
Step 0 made three separate round-trips (state.load, resolve-model
gsd-debugger, config-get workflow.tdd_mode) to assemble one context. Because
no debug-scoped fact was computed at any entry point, ADR-1671 admission gate
(2) could never be satisfied for debug — an applicability atom naming such a
fact would evaluate FALSE forever and silently exclude its section.

Adds cmdInitDebug (init.debug), registers it in the init router and the
command-alias table, and collapses debug.md Step 0 to one call. Every field
resolves through the same primitive the call it replaces used: loadConfig for
commit_docs, withProjectRoot for response_language (#2402), planningPaths for
debug_dir, resolveModelInternal for debugger_model, and the existing
Boolean(workflow.tdd_mode) idiom for tdd_mode.

PlanningPaths gains a debug field so state.load and init.debug share ONE
debug-directory expression rather than two kept in sync by hand. state.load
keeps emitting debug_dir: it is a shipped query surface with its own test
anchor, so narrowing it would break unseen consumers for no gain.

No WHEN_VOCABULARY atom and no gsd:section marker: gate (1), a consuming
section of at least 400 bytes, belongs to the change that adds the section.

d626dbc6e3 fix(#1883): distinguish a permission/IO error from genuine emptiness in dir scans (#2802)
* test(#1883): failing-first regression for findContextMdIn / listMilestoneArchiveDirs swallowing EACCES

Adds failing-first regression tests proving an unreadable dir is currently
swallowed as empty/null instead of surfacing the permission error. Covers
EACCES, EIO, the unchanged ENOENT empty path, the array fast-path, and both
CONTEXT.md forms. listMilestoneArchiveDirs is exercised in-process via a new
_listMilestoneArchiveDirs test seam (the validate command runs in a subprocess,
so an fs monkeypatch in the test process cannot reach it).

* fix(#1883): distinguish a permission/IO error from genuine emptiness in dir scans

findContextMdIn (src/planning-workspace.cts) and listMilestoneArchiveDirs
(src/verify.cts) catch-alled every readdirSync error into the empty marker
(null / []), conflating a genuine ENOENT ('nothing there') with an EACCES/EIO
failure ('can't read this'). An unreadable phase dir was silently reported as
'no CONTEXT.md' (discuss/plan gates wrongly skipped context) and an unreadable
milestones/ dir as 'no archives' (active-milestone resolution / archived-phase
filtering misbehaved).

Narrow each catch to ENOENT only — keep the long-standing null/[] contract for
genuine absence (Hyrum: empty path unchanged) and re-throw every other error so
it propagates to the caller's existing try/catch. All six findContextMdIn
callers either pass a pre-read string[] (no readdir) or sit inside a try block
that already handles readdir failures; the two listMilestoneArchiveDirs callers
live in the validate command path where errors reach the command error handler.

Exposes a _listMilestoneArchiveDirs test seam so the permission-error path can
be unit-tested in-process (the validate command runs in a subprocess, so an fs
monkeypatch in the test process cannot reach the private helper).

* fix(tests): delete stale emitted-drift ack for gsd-phase-researcher.md

Pre-existing base-branch defect, not part of #1883: commit 6932fb16d
(enhance(#1699): require read-and-cite provenance, #2768) landed the
gsd-phase-researcher.md growth onto next WITHOUT removing its now-obsolete
emitted-drift-ack.json entry. An ack explains a ripple in a PR diff; once the
change merges to next the ripple becomes part of the baseline, so the ack is
permanently stale for every subsequent PR — the emitted-attribution gate fails
on every branch off next with '1 stale acknowledgment(s) — the ripple they
explained is gone'.

The gate's own error message prescribes the fix: delete the stale entry, and
since gsd-phase-researcher.md was the only entry, delete the file (an empty ack
file signals nothing; its presence is the alarm). This unblocks the red base for
all in-flight PRs, not just this one. Folded inline per the bug-fixer directive
(one-line test-prescribed fix, does not bury the #1883 fix).

* docs(changeset): #1883 permission-error-not-empty

* test(#1883): use t.mock.method + t.after per CONTRIBUTING test conventions

Address code-review finding: replace inline try/finally in test bodies and
manual fs.readdirSync reassignment with t.mock.method (auto-restored) and
t.after for tmp-dir cleanup, matching CONTRIBUTING.md test patterns and the
existing t.mock.method idiom in tests/phase.test.cjs.

51dfa683d4 fix(#2028): phase.complete milestone-end out-of-order + workstream root-fallback guard
Two code-confirmed defects in `gsd-tools phase complete` (re-verified against
next; the three severe corruption paths the issue filed are superseded by the
ADR-1769 Transition Module migration + #2012, so this is the confirmed remainder).

1. Milestone-end mislabel (isLastPhase). The milestone-end determination only
   cleared isLastPhase when a HIGHER-numbered phase existed, so completing the
   numerically-highest phase out of order (e.g. Phase 10 before Phase 9) stamped
   STATE.md `Status: Milestone complete` while a lower phase was still outstanding.
   Added a lower-phase check: after the existing higher-phase scans, if any earlier
   phase in the current milestone has an unchecked roadmap checkbox (`[ ]`),
   isLastPhase becomes false AND next_phase/next_phase_name point at the LOWEST
   outstanding lower phase — so STATE.md advances to the real gap instead of
   parking on the just-completed phase. A completed phase always has `[x]`
   (phase.complete sets it), so all-lower-complete still reports milestone-end;
   heading-only roadmaps (no checkboxes) retain prior behavior. The checkbox regex
   mirrors the sibling phasePattern's anchoring (whitespace/bold + required `:`) so
   unrelated checklist lines mentioning "Phase N" don't match.

2. Workstream root-fallback (no guard). cmdPhaseComplete resolves every path via
   planningDir(cwd); with a `workstreams/` dir present but no active workstream and
   no --ws, that returns root `.planning`, so phase.complete wrote STATE.md/
   ROADMAP.md (and the mislabel) into the shared root other workstreams read.
   Added the same #1912 fail-safe guard init.progress got: refuse (asking for
   `--ws`/active workstream) instead of silently writing root. Resolution itself
   was already wired globally (resolveActiveWorkstream: --ws > GSD_WORKSTREAM >
   pointer, set in bin/gsd-tools.cjs), so only the refusal guard was missing.

The workstream-mode detection (`listAvailableWorkstreams`) is extracted into
planning-workspace.cts as the single source of truth and consumed by BOTH
init.progress and phase.complete, so the two fail-safe paths cannot drift.

Tests (tests/phase.test.cjs, new #2028 describe): out-of-order completion becomes
`Ready to plan` with is_last_phase=false, next_phase pointing at the outstanding
phase and Current Phase advancing to it (not the completed phase); all-lower-
complete still reports milestone-end; workstream-mode-no-active refuses with an
`--ws` hint; `--ws` completes in the workstream leaving root untouched; flat mode
unaffected. Fail-first verified locally via direct gsd-tools invocation.

871621c3c8 feat(#1740): require-fs-op-fallback production AST rule + Windows transient-lock retry (Phase 6) (#1742)
* feat(#1740): require-fs-op-fallback production AST rule + Windows transient-lock retry (Phase 6)

ADR-1703 Phase 6 of the cross-platform portability epic (#1702). Adds the
second production-code portability AST rule + the ADR-mandated glob expansion
to bin/install.js and scripts/build-hooks.js.

- eslint-rules/require-fs-op-fallback.cjs: flags an unguarded fs.rename /
  fs.renameSync (the atomic-publish primitive named first in
  DEFECT.WINDOWS-FS-OPS.symptom) that is NOT inside a try/catch whose handler
  references a transient errno ('EPERM'/'EBUSY'/'EACCES' or a *RETRY_ERRNOS
  set) AND NOT behind a Windows platform guard. A catch that silently swallows
  or cleans-up-and-rethrows without an errno check does NOT satisfy the
  defect's 'never silently swallow' clause. copyFile/unlink are deliberately
  not flagged (they are the fallback primitives per the defect's own
  fix-forward). Scope narrowed to rename per Phase 5's precision discipline;
  documented on #1740.

- src/shell-command-projection.cts: export retryRenameSync(from, to) — the
  drop-in bounded-retry helper over the existing atomicRenameWithRetry.

- 27 bare fs.renameSync sites across 11 modules routed through retryRenameSync
  (capability-lifecycle/lock/source, installer-migrations, milestone, phase,
  planning-workspace, roadmap-upgrade, runtime-hooks-surface, state,
  workstream). Idempotent on POSIX; resilient to AV/indexer transient locks
  on Windows.

- eslint.config.mjs: register rule at error on src/**/*.cts; new focused
  portability-rules block covering bin/install.js + scripts/build-hooks.js
  (ADR-1703 L124-126 glob expansion — both files are compliant: zero
  rename violations).

- tests: 15-case RuleTester suite; portability-rule-disable-ban extended
  (PROTECTED_RULES + scans bin/install.js/build-hooks.js with shebang
  handling); ci-test-scope portability-lint selection rule.

- CONTEXT.md DEFECT.WINDOWS-FS-OPS predicate rewritten to point at the rule;
  docs/contributing/cross-platform-portability-rules.md reference + how-to.

Closes #1740

* chore(#1740): backfill changeset pr:1742

* fix(#1740): tighten require-fs-op-fallback precision (codex review HIGH-1/HIGH-2)

Addresses two false-negative findings from the codex (gpt-5.5/high)
adversarial review of PR #1742:

HIGH-1 — a catch that REFERENCES a transient errno but only rethrows (no
retry/fallback) was marked compliant. The DEFECT.WINDOWS-FS-OPS fix-forward
requires retry, not just recognition. Fix: catchHandlerHasRetrySignal now
requires a loop `continue` backedge OR a `return <call>` delegation; a bare
rethrow is flagged. The misleading `/* retry logic */` valid test is replaced
with a real retry loop, and the rethrow-only shape is added as invalid.

HIGH-2 — the nested-try ancestor walk treated an OUTER errno-catch as
protecting the rename even when an INNER catch intercepted/swallowed the error
(the outer catch is unreachable). Fix: isInsideTransientErrnoTryCatch now stops
at the NEAREST enclosing TryStatement WITH A CATCH HANDLER whose block contains
the rename (try-finally is skipped — it doesn't catch); outer catches are no
longer consulted. The unsound nested-try valid test is converted to invalid,
and a try-finally-skipped valid case is added.

Verified: 17 RuleTester cases pass; zero new production violations (the 27
fixed sites use retryRenameSync; the real retry loops — atomicRenameWithRetry,
capability-ledger/consent, build-hooks — remain compliant via continue/errno);
lint:ci green; disable-ban + vocab-drift green.

fc019b2688 fix(#1531): race-safe steal for the two core-path locks (PR #1532 review)
trek-e's review found the M1 PID-liveness backport dropped two pieces of
capability-lock.cts's steal-safety machinery, reopening the #500/#905/#1230
lost-update family:

- Empty-body window (state.cts): acquireStateLock creates the lock with O_EXCL
  and writes the pid in a separate writeSync; a lock observed in that gap has an
  empty body, reads as not-verified-live, and was stolen at age ~0 — robbing a
  holder mid-creation. Add a fresh-create floor scoped to the unverifiable-body
  case: an empty/unparseable body that is fresh is treated as mid-creation and is
  NOT stolen, while a COMPLETE dead-pid body is still stolen promptly (preserves
  the prompt-dead-steal contract). planning-workspace writes its body atomically
  (flag:'wx') so it has no empty-body window.

- Double-steal (both locks): the steal was a bare fs.unlinkSync with no identity
  re-confirm, so two waiters could both reclaim a dead holder and end up holding
  concurrently. Replace with an atomic renameSync (only one racer wins the inode)
  guarded by a (dev,ino,body) identity re-confirm immediately before the steal;
  body content is part of the identity to defeat inode reuse.

Tests (seam-driven, no wall-clock, each proven RED-before-GREEN):
- clock-seam: fresh empty-body lock is not stolen at age ~0; a racer-recreated
  live lock is not double-stolen (identity re-confirm). Adds a beforeSteal seam.
- planning-workspace: racer-recreated live lock is not double-stolen.
- Updated the two #1217 unlinkSync-failure tests to the renameSync steal path
  (the bounded-backoff/no-busy-spin guarantee is preserved and re-asserted).

Uncontended acquire path is byte-for-byte unchanged.

b2602cc61d fix: add deadman ceiling to withPlanningLock (M1/M2 R4-FIX asymmetry)
The M1/M2 lock fix (4903ee04) was asymmetric: acquireStateLock got a 60s deadman
ceiling (recovers a lock once its age crosses an absolute bound ABOVE the wait
budget) but withPlanningLock did not. The .lock body carries no startTime, so
_planningHolderVerifiedLive can only check pid liveness — it cannot detect pid
reuse. A false-alive holder (original holder crashed, pid recycled by an unrelated
live process) would therefore make withPlanningLock throw on every call with no
self-heal until the reused pid happens to die.

Mirror acquireStateLock: in the EEXIST branch, steal a verified-live holder anyway
once the lock ages past deadmanCeilingMs (60000 > lockTimeout 10000). mtime age is
measured from lock creation, so a stuck lock self-heals on a subsequent call.

Adds a clock+probe-seam regression test (false-alive holder past the ceiling IS
stolen). planning-workspace 13/13, clock-seam 34/34, locking suites green; lint clean.

fc9bd70aff fix: PID-liveness gate for the two core-path file locks (audit M1+M2)
The STATE.md write lock (acquireStateLock) and the .planning/ workspace lock
(withPlanningLock) stole contended locks on mtime age alone with no
process.kill(pid,0) liveness check, and mis-ordered stale-vs-wait so a
live-but-slow holder could be robbed mid-critical-section.

M1 (lost update / STATE.md corruption): a live writer whose critical section
ran past the stale threshold aged out and a waiter unlinked its lock and
acquired -> two writers in STATE.md's read-modify-write window. mtime is a
leaky proxy for "holder is alive"; it leaks under exactly the slow-holder
condition the lock guards against.

M2 (uncaught EEXIST): withPlanningLock's timeout fallback unconditionally
unlinked whatever lock existed (even a live holder's) and re-acquired OUTSIDE
any try -- a concurrent re-create raced a raw EEXIST out of the helper.

Fix backports capability-lock.cts's liveness gate (process.kill(pid,0) via a
_setLockProbes/_resetLockProbes test seam):
- acquireStateLock: steal when holder pid is DEAD (any age) OR age exceeds a
  deadman ceiling (60000ms, ABOVE maxWaitMs=30000) so a verified-live holder is
  never stolen within budget; garbage/legacy bodies stay recoverable.
- withPlanningLock: same gate in the EEXIST path (dead stolen promptly, live
  waited on); removed the unconditional force-steal -> clear timeout throw,
  which also closes M2 (no re-acquire outside try).

Uncontended path unchanged byte-for-behaviour; realClock + real process.kill
remain the defaults. Pid-reuse residual fails safe (waits/times out, never
corrupts) and recovers at the deadman ceiling.

Tests: TDD red->green via the clock + new pid-liveness probe seams (no
wall-clock; #453 deleted the race tests). 8 new behavioural tests across
tests/clock-seam.test.cjs and tests/planning-workspace.test.cjs; the prior
withPlanningLock timeout test rewritten to pin the no-force-steal contract.

df04aae5e4 enhancement(#537): migrate all hand-written bin/lib/*.cjs to TypeScript source of truth (ADR-457) (#602)
* enhancement(#537): migrate code-review-flags to TS source of truth

Collapse the hand-written get-shit-done/bin/lib/code-review-flags.cjs to a
TypeScript source of truth (src/code-review-flags.cts), compiled by tsc to a
gitignored .cjs build artifact at the same path, per ADR-457 (build-at-publish).
Second module after the semver-compare pilot (#541).

Behaviour is preserved byte-for-behaviour (characterization test added in
tests/code-review-flags.test.cjs locks the parser quirks). Adds compile-time
type checking: CodeReviewFlags interface + CodeReviewWorkflow literal union.
The require() path is unchanged, so code-review.md and the bug-3727 test keep
working. The emitted .cjs is gitignored and eslint-ignored, mirroring the pilot.

* enhancement(#537): migrate 9 leaf bin/lib modules to TS source of truth

ADR-457 build-at-publish, batch 1 (pure leaf modules, 0 sibling-deps):
001-legacy-orphan-files, context-utilization, redaction, artifacts,
command-arg-projection, clock, ui-safety-gate, review-reviewer-selection,
clusters. Each moves to src/*.cts (strict TS, typed), compiled by tsc to a
gitignored .cjs at the same require() path; behaviour preserved byte-for-
behaviour. Adds src/node-globals.d.ts (minimal ambient shim; "types":[]).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* chore(#537): add @types/node, drop hand-rolled node-globals shim

ADR-457 migration infra: replace the temporary src/node-globals.d.ts ambient
shim with @types/node@22 + "types":["node"] in tsconfig.build.json. Unblocks
migrating the ~49 remaining bin/lib modules that use node:fs/path/os/
child_process. Build + full suite (3030 pass) + lint all green; no .cts type
changes were needed (real Node types matched the shim).

* enhancement(#537): migrate 9 more bin/lib modules to TS (batch 2)

ADR-457 build-at-publish. Clean leaves: installer-migration-report,
prompt-budget. Type-error-prone leaves (were tsconfig.lint-excluded; now
strict-typed and removed from that exclude list): secrets, phase-lifecycle,
workstream-name-policy, decisions, validate, schema-detect. Plus
runtime-name-policy. Strict type fixes narrow unknown->concrete domain types
(no any/ts-ignore); behaviour preserved. Full suite green, lint 0 errors.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate runtime-slash to TS (cross-import proof)

ADR-457. First cross-module TS->TS import: src/runtime-slash.cts imports
./runtime-name-policy.cjs and tsc resolves the sibling .cts types under strict
(no declaration files; NodeNext .cjs->.cts mapping), emitting a correct
require("./runtime-name-policy.cjs"). Confirms the recipe for coupled modules,
which must be migrated in dependency order (leaves-up). Suite green, lint clean.

* enhancement(#537): migrate 10 more bin/lib modules to TS (batch 3)

ADR-457 build-at-publish, Wave-1 leaves: event, workstream-inventory-builder,
plan-scan, fallow-runner, project-root, installer-migration-authoring,
update-context, 000-first-time-baseline, runtime-homes, model-catalog. Strict
typing fixed real issues (narrowing unknown, qualified fs/path calls, removed
unnecessary casts); plan-scan/project-root/workstream-inventory-builder dropped
from tsconfig.lint exclude. Behaviour preserved; suite green, lint 0 errors.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate 5 large Wave-1 leaves to TS (batch 4)

ADR-457 build-at-publish: configuration, state-document, shell-command-
projection (42 dependents), security, command-aliases. shell-command-
projection keeps a namespace child_process import for mock-intercept
testability. loadConfig/migrateOnDisk emit synchronously (every caller uses
them sync; the one awaited migrateOnDisk caller tolerates a non-Promise) —
full suite (3030 pass) confirms behaviour preserved. configuration/
state-document/command-aliases dropped from tsconfig.lint exclude. Also fixes
the malformed batch-3 changeset frontmatter (type/pr) that failed lint:docs.

* enhancement(#537): migrate 6 Wave-2 modules to TS (batch 5)

ADR-457 build-at-publish: config-schema, model-profiles,
002-codex-legacy-hooks-json, logger, active-workstream-store, adr-parser.
First batch importing already-migrated siblings (configuration, model-catalog,
shell-command-projection, redaction, security) via ./sibling.cjs specifiers.
Strict type narrowing (typeof guards over String(unknown)); behaviour
preserved; suite 3030 pass, lint 0 errors.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate 5 large Wave-2 modules to TS (batch 6)

ADR-457 build-at-publish: graphify, install-profiles, intel,
installer-migrations, worktree-safety. installer-migrations preserves its
dynamic require() loader for numbered migration modules (scoped lint
suppressions). Strict typing (typeof guards over String(unknown)); behaviour
preserved; suite 3030 pass, lint 0 errors. Wave 2 complete.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate Wave-3 modules to TS (batch 7)

ADR-457 build-at-publish: planning-workspace, runtime-artifact-layout,
command-routing-hub, drift. Uses `import x = require()` for export= siblings;
drift's lazy require of runtime-slash hoisted to a top-level import (verified
non-circular). Behaviour preserved; suite 3030 pass, lint 0 errors.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate small Wave-4 modules to TS (batch 8)

ADR-457 build-at-publish: cjs-command-router-adapter, phase-command-router,
surface, roadmap-upgrade. Typed the hub router handler results as the HubResult
discriminated union; surface drops 4 genuinely-unused imports. Behaviour
preserved; suite 3030 pass, lint 0 errors.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate core hub (2.5k LOC, 68 dependents) to TS (batch 9)

ADR-457 build-at-publish: get-shit-done/bin/lib/core.cjs -> src/core.cts,
preserving all 63 exports via export=. All sibling deps already migrated
(shell-command-projection, model-profiles, model-catalog, worktree-safety,
planning-workspace, project-root, configuration, config-schema). Strict types,
no any/ts-ignore; config-schema lazy require hoisted (non-circular). Behaviour
preserved (independently verified: core's shard 3030 pass / 0 fail).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* test(#537): make ESLint-coverage + test-sprawl checks migration-aware

#551 test hardcoded 12 now-migrated modules as "hand-written, must be linted";
that invariant is obsoleted by the ADR-457 migration. Rewrite it to a
filesystem-driven invariant that holds at every stage: a bin/lib/*.cjs must be
eslint-ignored IFF it has a src/*.cts source (tsc-generated), else linted
(covers package-identity, which has no TS source). Also eslint-ignore
config-types.cjs (has a src counterpart) and drop the redundant
tests/clock.test.cjs (clock already covered by clock-seam + bug-474 tests),
which tripped the lint-test-file-count ratchet.

* enhancement(#537): migrate 9 Wave-5 router/inventory modules to TS (batch 10)

ADR-457 build-at-publish: phases/verify/init/agent/task/validate/roadmap/state
command routers + workstream-inventory. Router handler results typed against
core's exported shapes; behaviour preserved (caught+fixed a --verify boolean
flag regression mid-migration). Full suite green across all shards (only the 4
local gpg-env changeset-notes failures remain; CI passes them).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate 7 Wave-5 modules to TS (batch 11)

ADR-457 build-at-publish: gap-checker, docs, check-command-router, frontmatter,
learnings, gsd2-import, profile-pipeline. Behaviour preserved; full suite green
across all shards (only the 4 local gpg-env failures remain). Also broadens
atomic-write-coverage.test.cjs to accept the tsc-compiled namespace-import form
while still asserting platformWriteSync is called (safety guard intact).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate config + profile-output to TS (batch 12)

ADR-457 build-at-publish: config (729 LOC), profile-output (1142 LOC). All
exports preserved; cmdMigrateConfig de-asynced (migrateOnDisk is sync, awaited
caller tolerates it). Behaviour preserved; suite green across all shards
(only the 4 local gpg-env failures). Wave 5 complete.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate 5 Wave-6 modules to TS (batch 13)

ADR-457 build-at-publish: template, uat, workstream, roadmap, audit. Behaviour
preserved (dead toPosixPath import dropped from audit; inline requires hoisted).
Suite green across all shards (only the 4 local gpg-env failures).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate commands + state hubs to TS (batch 14)

ADR-457 build-at-publish: commands (1305 LOC), state (2074 LOC, 17 dependents).
All exports preserved; inner requires kept non-hoisted where load-order matters
(install.js, per-call security); acquireStateLock cast inlined to preserve the
err.code source token a structural test inspects. Behaviour preserved; suite
green across all shards (only the 4 local gpg-env failures). Wave 6 complete.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate milestone to TS (batch 15a, hand-authored)

ADR-457 build-at-publish: milestone -> src/milestone.cts. Authored directly
(subagent capacity was unavailable). Also relaxes core.output()'s 3rd param to
optional, matching its real always-optional call contract (unblocks remaining
2-arg output callers). Behaviour preserved; suite green across all shards
(only the 4 local gpg-env failures).

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537): migrate phase, verify, init to TS (batch 15, final modules)

ADR-457 build-at-publish, Wave 7 (the last hubs): phase (1608 LOC), verify
(1615), init (2113). Adds src/package-identity.d.cts so verify can import the
permanently value-baked package-identity.cjs under strict TS.

Fixes two regressions the migration introduced in verify: restore
cmdValidateHealth's `return result` (callers/tests read result.warnings — it is
NOT side-effect-only), and make the bug-3384 source-pattern test tolerant of the
tsc-compiled bracket-notation form of the git_list_failed->W020 branch (behaviour
intact). Full suite green across all shards (only the 4 local gpg-env failures);
lint 0 errors. All 86 migratable bin/lib modules are now TypeScript sources.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* chore(#537): finalize ADR-457 migration — retire tsconfig.lint.json

All hand-written bin/lib/*.cjs are now src/*.cts sources, so the checkJs
stopgap tsconfig.lint.json (unused; not wired into eslint, scripts, or CI) is
deleted per ADR-457's final step. Also gitignore the tsc-generated
config-types.cjs (was still committed) for consistency with every other
emitted artifact. package-identity.cjs stays value-baked (declared via
src/package-identity.d.cts). Suite green; #551 ESLint-coverage test green.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#537): add prepare script so unpacked/git installs build bin/lib artifacts

ADR-457 build-at-publish: bin/lib/*.cjs are now gitignored, built by tsc. The
prepack/prepublishOnly hooks cover `npm pack`/publish, but `npm install -g
<dir>` and git installs run the `prepare` lifecycle — which was missing — so the
unpacked install shipped without the compiled .cjs and failed at startup with
"Cannot find module './lib/core.cjs'" (caught by the smoke-unpacked CI job).
Add `prepare` mirroring prepublishOnly (build:lib + build:hooks). prepare does
NOT run for registry consumers (they get the pre-built tarball), only for
source/local/pack installs.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#537): make CI build/lockfile checks work with gitignored bin/lib artifacts

ADR-457 build-at-publish exposed two CI assumptions that bin/lib/*.cjs are
always present on disk:
- check:env's lockfile-sync ran `npm ci --dry-run`, which now triggers the
  `prepare` build (tsc) — but it runs before deps are installed, so tsc is
  absent and it misreported the lockfile as out of sync. Add --ignore-scripts
  (a lockfile check must not build).
- the lint-tests job installs with --ignore-scripts (no prepare build), but
  lint:skill-deps require()s the built install-profiles.cjs. Add an explicit
  `npm run build:lib` step after install.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#537): narrow prepare to build:lib only (unbreak packed-smoke pack step)

prepare running build:hooks emitted "✓ Copying ..." stdout during `npm pack`,
which the install-smoke "Pack root tarball" step captures into $GITHUB_OUTPUT —
breaking it with "Invalid format". build:lib (tsc) is silent on success and is
all the unpacked/source install needs (the smoke-unpacked assertions exercise
gsd-tools, i.e. bin/lib, and tolerate hook setup with `|| true`). Matches
prepack. build:hooks still runs on prepublishOnly for real publishes.

* fix(#537): wire Stryker mutation gate to build-at-publish layout

The gate scored 0.00 because it mutated changed bin/lib/*.cjs that (a) were
generated artifacts and (b) included modules with no coverage in the command's
test set. Rework: mutation.yml now derives changed COVERED modules from
src/*.cts and maps them to their built bin/lib/*.cjs; Stryker mutates those
built artifacts with a no-rebuild command (mutating src/*.cts + per-mutant tsc
was ~3x over the 30-min CI budget).

NOTE: with the gate now correctly measuring the covered modules, their actual
mutation score is 42.94% (< break 50) — a pre-existing test-coverage gap
(adr-parser/prompt-budget/etc.), not introduced by this behaviour-preserving
migration. Reaching 50 needs more tests, a threshold/scope change, or a waiver —
a maintainer decision.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* test(#537): raise mutation coverage of covered modules above the 50 gate

Adds focused example-based unit tests that kill surviving mutants in the two
lowest-scoring covered modules:
- tests/prompt-budget.unit.test.cjs (112 tests): 17.9% -> 97.9%
- tests/adr-parser.unit.test.cjs (205 tests): 44.7% -> 89.4%
Both wired into stryker.config.mjs's command. Fresh full run over the 6 covered
modules now scores 82.25% (>= break 50); every covered module is >= 68%.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* enhancement(#537,#609): parallelize mutation gate via dynamic per-module matrix

The serial Stryker run timed out at 30 min once the migration's added tests
made every mutant re-run ~300 tests. Replace it with a dynamic matrix so the
gate completes well under budget — folded into this PR (was tracked as #609)
because it's a prerequisite for this PR's mutation gate to pass.

- scripts/mutation-matrix.cjs: single source of truth (covered-module -> test
  files) computing changed covered modules from git diff -> {has_work, matrix}.
- mutation.yml: detect -> dynamic `matrix: fromJSON(...)` mutate job (one
  parallel shard per changed module, scoped via MUTATION_TEST_CMD to only that
  module's tests, 15-min/shard) -> summary job that KEEPS the legacy check name
  "Stryker mutation score (changed files only)" so branch protection is
  unchanged. Per-shard jobs report as "Stryker (<module>)".
- stryker.config.mjs: commandRunner.command reads MUTATION_TEST_CMD (falls back
  to the full command locally).

Closes #609.

* test(#537,#609): give each mutation shard ≥50% on its own tests; drop blacksmith note

Per-module sharding revealed that active-workstream-store (46.5%) and
frontmatter (7.4%) only cleared 50% in the old serial run via timeout-noise from
the bloated 300-test command; on their own tests they were below the gate. Add
focused unit tests:
- tests/active-workstream-store.unit.test.cjs (115 tests): 46.5% -> 81.9%
- tests/frontmatter.unit.test.cjs (165 tests): 7.4% -> 63.4%
Both wired into scripts/mutation-matrix.cjs (per-module test map) and
stryker.config.mjs DEFAULT_TEST_CMD. All 6 covered modules now clear break:50
with only their own tests (config-schema/context-utilization/prompt-budget/
adr-parser already did). Also removes the leftover blacksmith TODO comment —
GitHub-hosted runners only; speed comes from parallel per-module shards.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* test(#537,#609): strengthen prompt-budget tests to clear the gate on its own tests

prompt-budget scored 39.58% when mutation-tested with ONLY its own tests (the
way the per-module CI shard runs it) — an earlier ~98% reading was inflated by
accidentally running the full multi-module command. Add 96 targeted tests to
tests/prompt-budget.unit.test.cjs (exact note-template text, plan-truncation
arithmetic/percentages, drop-block strings, noteInjected/hardFailed booleans):
scoped score 39.58% -> 68.75% (>= break 50). All 6 covered modules now clear
the gate on their own tests.
