The stale check after UAT used phase_completion.verification_status from
session-start INIT while human_needed promotion already queried fresh
verification.status. Align the stale gate with the canonical query so
mid-session verification refresh is not ignored.

* fix(init): skip roadmap-checked phases when selecting next_phase

Roadmap-only phases without a disk directory were still promoted to
next_phase when their checkbox was already checked. Exclude
checkboxComplete phases so progress routing does not point at work the
roadmap already marks done.

* fix: gaps_found not overridden by stale, transition uses canonical verification

- verification.cts: check gaps_found before stale so gap-closure routing
  is not masked by a newer summary mtime
- phase.cts: remove redundant findStaleVerificationSummary — readVerificationStatus
  already handles stale detection
- transition.md: replace raw grep on file content with verification.status query
  to avoid false-positive blocks from body text matching

The runtime launcher is not defined until the update_roadmap_and_state step
bash block (~line 165). The early verify_completion block used gsd_run to
query verification.status, which violated the runtime-launcher-parity test:
'preamble appears AFTER the first gsd_run reference'.

Replace the gsd_run call with an awk-based frontmatter extractor that reads
only the status: field between the two --- fences. This avoids both the
preamble-ordering constraint and the original false-positive grep bug where
body text like 'previous_status: gaps_found' would match a full-text regex.

The phase.complete gate at update_roadmap_and_state is the canonical
enforcement point; this early check is advisory only.

Also update workflow-size-baseline.json for the updated transition.md size.

cannot slip through when a SUMMARY.md is written between the gate and
the roadmap/state mutation. Return the blocked status from the lock

The awk-based check read raw frontmatter status: passed, which misses the
stale case where summaries are newer than the VERIFICATION.md file even
though the frontmatter still says passed. The stale status is computed from
file modification times, not stored in frontmatter.

Move the preamble to the verify_completion bash block (the first block with
a gsd_run call) so gsd_run query verification.status can be used for the
advisory check. This gives the full readVerificationStatus logic including
mtime-based staleness detection, matching the enforcement gate at phase.complete.

Capture full JSON (VERIFY_JSON) so next_action can be included in the
advisory output alongside the status.

Also update workflow-size-baseline.json for the updated transition.md size.

so the verification debt check at line ~80 cannot use gsd_run. Restore the
awk-based frontmatter extraction that correctly reads status without needing
the runtime, and restore the shim at its proper location before
phase.complete.

Address review blocker B1 on #1548: findStaleVerificationSummary ran fs.readdirSync
and two fs.statSync calls unguarded between readVerificationStatus's try/catch sections,
so a TOCTOU race (a SUMMARY listed by scanPhasePlans then removed before statSync) or any
FS error threw uncaught into callers NOT under the planning lock (init.manager /
init.progress / uat-predicate). Wrap the body in try/catch degrading to 'not stale', and
thread the injectable opts.fs seam (add statSync to FsLike, pass fsImpl from the caller)
for parity with readVerificationStatus's no-throw contract and testability. Also adds the
Verification Module glossary entry to CONTEXT.md (review B3).

After T0–T6 nothing imports core, so retire the spine and its scaffolding:
- delete src/core.cts (and the gitignored gsd-core/bin/lib/core.cjs artifact;
  remove its .gitignore + eslint-ignore entries)
- delete scripts/lint-core-spine-imports.cjs + its allowlist; drop it from the
  package.json lint:ci chain
- regenerate docs/INVENTORY-MANIFEST.json (drops the core.cjs surface)
- sweep stale references: CONTEXT.md glossary back-compat clauses (spine retired,
  callers import the leaf directly), planning-config.md CONFIG_DEFAULTS owner,
  and false present-tense core.cjs claims in leaf-module docstrings

The ADR-857 decomposition is complete: the former Core god-module is fully
dissolved into its leaf modules; no re-export spine remains. No behaviour change.

bin/install.js used require(path.join(_gsdLibDir, 'core.cjs')) (a computed
path, and bin/install.js was never in the convergence lint's scan roots), and
~8 test files referenced core.cjs via path.join/readFileSync/existsSync/FILE_ARG
forms the literal-string migration grep missed. Route install.js's symbols to
their leaves (RUNTIME_PROFILE_MAP->model-catalog, resolveTierEntry/EFFORT_SET->
model-resolver) and repoint/adjust the test references to the leaves. Recovers
the 161 'Cannot find module core.cjs' failures from the spine deletion.

* fix(#1008): tolerate EAGAIN + short writes in io output()/error()

The I/O Module wrote stdout/stderr with a bare fs.writeSync(fd, data), assuming
it blocks until the kernel accepts every byte. That is false when the fd is a
non-blocking pipe (as under the parallel node:test runner on Linux CI): a full
pipe throws EAGAIN and a partially-drained pipe returns a short count. The former
caused spurious failures (e.g. bug-974 graphify property test threw EAGAIN); the
latter risked silently truncating output.

Add writeAllSync(fd, data): loop on short counts and retry EAGAIN/EINTR with a
bounded backoff. The backoff sleep buffer is allocated lazily on the first retry
(rare) and reused — keeping it out of module load avoids perturbing the
SharedArrayBuffer-allocation accounting in perf-316 and costs nothing on the
common no-retry path. Route output() and error() through it; non-transient
errors (EPIPE) still propagate. Mirrors the transient-errno handling already
applied to STATE.md lock acquisition (ACQUIRE_LOCK_RETRY_ERRNOS / #3776).

Regression cases live in tests/io.test.cjs (the owning module's file, per the
regression-test-name placement policy) and inject fs.writeSync via mock.method:
EAGAIN/EINTR retry, short-write no-truncation, EPIPE still surfaces, and error()
retries while still exit(1). Red against the pre-fix bare-writeSync io.cjs.

a5d82bf98a refactor(#859): extract CLI I/O primitives from core.cts into io.cts (#864)
ADR-857 rollout phase 1. Move the CLI I/O primitives — output(), error(),
ERROR_REASON, setJsonErrorMode/getJsonErrorMode, and the output() large-payload
temp-file spillover helpers (GSD_TEMP_DIR, ensureGsdTempDir, reapStaleTempFiles)
— out of the 2271-line core.cts god-module into a new, small src/io.cts. core.cts
re-exports them so existing consumers are unaffected (behavior-preserving).

Repoint src/profile-pipeline.cts to import output/error/reapStaleTempFiles from
io directly; graphify/intel/audit were verified not to import these symbols. Net:
the leaf feature modules no longer depend on core just for I/O — the enabling
first cut toward Capability extraction.

New-CLI-module checklist: .gitignore (bin/lib/io.cjs), eslint.config.mjs ignores,
INVENTORY.md count 90→91 + io.cjs row, INVENTORY-MANIFEST.json, ARCHITECTURE.md
core.cjs/io.cjs rows, CONTEXT.md "I/O Module" glossary entry. Adds tests/io.test.cjs
(28 behavioral tests incl. shim-identity and the @file: spillover branch).

Gates: lint, code-review, security-review, codex adversarial-review, and
gsd-test-both (14742 pass on Mac + Linux Docker, 0 fail) all green.
