npx eslint . reported 0 errors, 53 warnings. The floor eroded the signal: a
genuinely new warning had to be spotted against noise, so 'lint is clean' was not
usable as a check. This takes it to zero.
Fifty of the 53 were one category in gsd-core/bin/gsd-tools.cjs — all unused
ARGUMENTS (error x30, cwd x10, raw x7, args x3), never unused variables. They are
the Command Routing Hub's uniform handler signature, function routeX({ args, cwd,
raw, error }), declared identically whether or not a handler uses all four
members. argsIgnorePattern: '^_' is structurally in conflict with that convention:
satisfying it would mean _-prefixing ~50 parameters and making the signature
non-uniform across the table. Option 1 of #2453: disable args checking for that
file only, keeping varsIgnorePattern intact so genuinely dead variables (the #2379
class) still surface — verified by injecting an unused variable, which still warns.
This is the config decision #732 explicitly deferred ('Severities stay warn (no
config change in this pass)').
The remaining three predate the issue's count of 51 and are real defects, not
suppressions:
- tests/workflow-compat.test.cjs — the step-9 lookahead used (?=\*\*10\.|\z).
\z is a Perl/Ruby end-of-input anchor with NO meaning in JavaScript; it matched
a literal 'z', so the lazy span silently stopped at the first z whenever **10.
was absent, truncating the captured step and letting the assertion pass against
a partial block. Corrected to $.
- tests/debugger-prevention.test.cjs — an unused RegExp built one line above the
one actually used. Removed.
- tests/installer-migrations.test.cjs — try/finally inside a test body, which
CONTRIBUTING prohibits ('verbose, masks test failures, not an approved
pattern'); the unused t was the symptom. Converted to t.after().

* test(#2470): failing-first — pi extension must satisfy pi's auto-discovery filter
pi auto-discovers extensions/ entries through isExtensionFile(), which accepts
only .ts and .js. GSD installs its extension as gsd.cjs, so pi silently skips
it: no /gsd command, no error, no log line.
Encodes pi's discovery PREDICATE rather than a literal filename, so the
contract keeps holding across future renames, and adds the migration-006 test
matrix for retiring the stale gsd.cjs left in pre-fix installs.
Red until the fix lands.

* fix(#2470): install pi's extension as gsd.js so pi actually discovers it
pi auto-discovers extensions/ entries via isExtensionFile(), which accepts
only .ts and .js and skips everything else silently. capabilities/pi declared
the dest as gsd.cjs, so the extension installed correctly and was then ignored
forever: no /gsd command, no error, no log line.
Install it as gsd.js. The in-repo source stays pi/gsd.cjs — tests require() it
directly and .cjs is unambiguous CommonJS; only the installed name has to
satisfy pi, and pi loads accepted files through jiti, which handles CJS and ESM
alike. (The reporter's premise that ~/.pi/agent/package.json declares
"type":"commonjs" does not hold — pi never writes that file.)
Renaming an installed artifact requires a migration record, so add 006 to
retire the stale gsd.cjs from pre-fix installs; without it the old path drops
out of the manifest and uninstall can never remove it. The migration plans
nothing for an unmanifested gsd.cjs: emitting remove-managed there would have
the executor downgrade it to preserve-user and mark it blocked, failing the
install for anyone who hand-placed their own file.
Also pins body-parser >=2.3.0 (GHSA-v422-hmwv-36x6). The advisory reaches the
production tree transitively via the Claude Agent SDK and fails the
npm-integrity gate, blocking any PR; pinned via the existing overrides idiom.

* fix(#2470): address orthogonal review findings + register migration checksum
Code review:
- pi/gsd.cjs's install docstring still told readers to copy the file to
extensions/gsd.cjs — the exact silently-broken state this PR fixes. Anyone
following it recreated the bug.
- Two stale extensions/gsd.cjs comments in install-minimal-hooks.test.cjs.
Security review:
- _installNativePluginIfDeclared confined nativePlugin.dir but joined
nativePlugin.file onto the validated directory unchecked, so a descriptor
whose file carried .., an absolute path, or a NUL byte would have written
outside configHome. Not reachable in a shipped build (descriptors are
first-party and compiled into the capability registry), but file is exactly
the field this PR changes. Confine the full dest path instead; for a
well-formed descriptor this resolves identically to the previous
mkdir(dir) + join(dir, file). Covered by four new write-confinement tests.
Also register migration 006 in the #670 EXPECTED_CHECKSUMS baseline — shipped
migration bodies are locked to a committed checksum and a new migration fails
CI until it is listed.

* fix(#2470): never dereference a symlinked managed path when snapshotting
fs.copyFileSync follows symlinks, so a managed path replaced by a link had the
REFERENT's bytes copied into the migration journal's rollback and backup trees
— a gsd.cjs symlinked at a private key would land that key's contents under
gsd-migration-journal/. Deletion was already safe (fs.rmSync unlinks the link,
never the target); the copy was not.
Nothing GSD installs is ever a symlink, so the faithful snapshot of a symlinked
managed path is the link itself. copyPreservingSymlink recreates it, which
keeps rollback fidelity (restore re-creates the same link) while never reading
the referent. Scoped the pre-delete to the symlink branch only, so the
regular-file path keeps copyFileSync's overwrite-in-place and a mid-restore
failure cannot destroy the destination. The restore-side existence check moves
to lstat, since existsSync follows a link whose target is gone and would
silently skip the restore.
This lives in the engine all six migrations share, so 000-005 are hardened too.
Also regenerates the pi golden-parity hash: correcting pi/gsd.cjs's own install
docstring changes the extension's content, which the golden suite caught.

* fix(#2470): symlink-preserve the in-apply failure-recovery restore too
The previous commit routed three copy sites through copyPreservingSymlink but
missed a fourth: the catch block inside applyInstallerMigrationPlan, which
replays rollback snapshots taken earlier in the SAME apply attempt. Those
snapshots are symlinks precisely because of that commit, so the raw
copyFileSync there dereferenced them and wrote the referent's bytes to the LIVE
install path — worse than the journal-tree leak it was meant to fix, since it
is user-visible and at a predictable location.
Verified by experiment rather than assertion: with the pre-fix line restored,
the managed path comes back as a REGULAR FILE containing the referent's bytes;
with the fix it comes back as a symlink and the bytes appear nowhere.
The accompanying test injects the failure by letting the delete succeed and
then throwing once, modelling a later step failing after the delete. That
ordering is load-bearing — an earlier draft injected before the delete, which
leaves the live path in place, so the pre-fix copyFileSync hit a same-file
collision and threw instead of leaking. That draft passed against the bug it
was written to catch; this one fails against it.
Adds the missing rollback() coverage as well: a restored symlinked managed path
must come back as a link pointing at its original target.

* test(#2470): read the backup location from the journal, not the plan
The new backup-content assertion read backupRelPath off result.plan.actions,
where it is always null: the planner reserves the field and apply chooses the
concrete location, recording it in the journal. The assertion therefore failed
on "backup path must be recorded for the user" rather than on anything about
the behavior it was written to check.
Read it from the journal, which is the authoritative record. Verified by
executing all four new test bodies in-process against the built engine — the
backup file exists and holds the locally patched content.

* test(#2329): fail-first tests for opencode commands/ (plural) command dir
Red phase, empirically probed: global/local install lands in command/ (singular)
with 71 gsd-*.md files and no commands/; the manifest records 71 keys under
command/ and zero under commands/; all four declaring sites report 'command'.
Migration coverage is black-box (two sequential install runs against one
configDir) so it holds regardless of how the fix implements cleanup.
The Kilo guard passes today by design — a forward-looking no-collateral check.

* fix(#2329): write opencode commands to commands/ (plural), migrate legacy command/
OpenCode discovers slash commands from commands/ (plural); the installer wrote
them to command/ (singular), so none of the ~71 /gsd-* commands appeared in the
TUI. Five sites declared the directory and all had to agree:
- capabilities/opencode/capability.json: both artifactLayout destSubpath entries
(global + local) and hostBehaviors.flatCommandDir
- bin/install.js: the manifest prefix was a SEPARATE hardcoded 'command/' literal,
so the manifest would have diverged from the descriptor even after a rename. It
now derives from _hostBehaviors(runtime).flatCommandDir.
- src/install-engine.cts installOpencodeFamilyArtifacts: the actual write target,
which bypasses resolveRuntimeArtifactLayout via combinedFamilyInstall. This was
a fifth site the issue did not list — without it the descriptor change alone
would not have moved a single file.
Migration: an upgrade over a pre-fix install removes only manifest-proven
GSD-managed files from the legacy command/ dir and rmdirs it once empty.
Unmanifested user files are preserved, never deleted.
Kilo shares the opencode family install path and is explicitly unaffected —
pinned by a no-collateral test.
Note on the tests: the migration cases originally built their legacy fixture by
running the installer and relying on it to produce command/ — i.e. they depended
on the bug to set up the fixture, and became unsatisfiable the moment it was
fixed (block 1 requires command/ to be absent after a fresh install). They now
fabricate the legacy layout explicitly, including rewriting the manifest keys to
the command/ prefix — which is load-bearing, since the migration only removes
manifest-proven files and an unrewritten fixture would silently no-op and pass
even against a broken migration.

* test(#2329): regenerate opencode install golden after rebase onto next
The golden conflicted on rebase because #2322 also regenerated it. Resolved by
regenerating from the merged source rather than hand-merging a generated file;
the only delta is the 71 command/gsd-*.md -> commands/gsd-*.md key renames.

* test(#2329): update stale tests that pinned opencode's singular command/ dir
Seven tests encoded the old contract (opencode: command/gsd-help.md exists, the
descriptor's flatCommandDir, the install-integration contract, and the
resolveRuntimeArtifactLayout golden). They passed in the red phase precisely
because they pinned the buggy singular dir; the fix intentionally changes that
contract, so these are stale-test corrections, not regressions.
Kilo shares the opencode family install path and is deliberately NOT changing —
it stays on command/ (singular). The shared opencode/kilo test is now split via
an explicit per-runtime dir map so the two cannot be conflated, and Kilo's own
layout test is untouched. tests/opencode-command-dir-plural.test.cjs
independently pins Kilo unchanged end-to-end.

* docs(#2329): correct the changeset — do not assert opencode ignores command/
The changeset repeated the issue's stated mechanism ("OpenCode discovers them
from commands/ ... a clean install produced no usable commands in the TUI at
all"). OpenCode's source contradicts that: packages/core/src/v1/config/command.ts
globs {command,commands}/**/*.md, so BOTH names resolve, and its own skill doc
still calls .opencode/command/ typical. Shipping that claim as a release note
would document a mechanism that does not exist.
The change is still right, for the stronger reason: OpenCode's config docs list
plural as the convention and singular as backwards compatibility, so GSD was
shipping on the alias the vendor may withdraw. Reworded to describe it as the
alignment it is, decided on OpenCode's source and docs rather than on bug reports
in either repo.

* fix(#2329): baseline opencode's commands/ surface — closes a data-loss path this PR opened
Not a bookkeeping gap. Moving opencode's command dir to commands/ moved the
install destination to a surface the first-time baseline scan does not cover:
000-first-time-baseline's RUNTIME_SURFACES.opencode lists ['gsd-core','command',
'skills','agents'] — no 'commands'.
installOpencodeFamilyCommands unconditionally unlinks every gsd-*.md under its
destination before writing the fresh set (install-engine.cts:870-873), with zero
manifest or migration involvement. The only thing that protects a pre-existing
file is assertInstallerMigrationsUnblocked, which runs before materialization and
halts when the baseline scan flags an unknown file at a KNOWN surface.
Probed: a pre-existing commands/gsd-plan.md is silently destroyed (install exits
0). The identical file under the legacy, already-baselined command/ surface
correctly halts the install with "installer migration blocked pending user
choice". So this PR would have traded a protected surface for an unprotected one.
Fixed with a NEW fix-forward migration rather than editing 000, per
docs/installer-migrations.md:131-134 — an applied migration never re-runs, so
editing 000 would only protect fresh installs and leave every existing machine
exposed. A new id runs for both populations and drifts no shipped checksum;
adding its entry to EXPECTED_CHECKSUMS is the case that test explicitly sanctions.
All five pre-existing shipped checksums verified byte-identical.
Kilo is excluded by the migration's runtimes filter and keeps command/.
This was previously deferred as a PR-body note claiming "low impact — nothing
else acts on baseline-scan misses". That claim was never probed and was wrong.

The product-name purity guard (#1777) rejects "Kilo (which still uses

is unchanged.

Drive Codex install/uninstall through the descriptor-driven Host-Integration
Interface (declarative embedding adapter → engine surface dispatch) and fold
every positive `runtime === 'codex'` / `isCodex` projection into descriptor-driven
`runtime.hostBehaviors`. Install/uninstall output stays byte-parity-gated
(tests/fixtures/golden-install-parity/codex.json); no other runtime changes.
Three Context7-verified upgrades, each with a test on the user-reachable surface:
- Skill root → canonical $HOME/.agents/skills via a skills-kind `home` override,
with pre-move migration cleanup (stale ~/.codex/skills/gsd-* removed on install
and uninstall; user content preserved). Fixes getGlobalSkillsBase, writeManifest,
and the skill-manifest inventory to honor the override so --skills-root /
sync-skills / the manifest report the real location.
- Six new hooks.json lifecycle events (PreToolUse, PermissionRequest, PreCompact,
PostCompact, SubagentStop, UserPromptSubmit) shared by install + uninstall;
extendedHookEvents reconciled [] -> the schema-valid wired subset.
- Explicit `[agents] max_depth = 1` in the managed config.toml block, pinning the
negotiated dispatch.maxDepth:1 axis. validateCodexConfigSchema now permits a
known-scalar-only bare `[agents]` AgentsToml table (still rejects [[agents]] and
unknown-key break-forms, #2760); mergeCodexConfig preserves the user's own
AgentsToml scalars (max_threads etc.) instead of dropping them.

Fold 27 issue-named installer/hooks/statusline/migration/reapply regression files
into their canonical module suites (installer-migrations, installer-migration-report,
gsd-statusline, reapply-verify-hunks, install-*, gsd-check-update-worker-platform-gate,
etc.). Verbatim block-scoped describe wrappers; 276 subtests conserved 1:1. No new files.
The one subdir origin (tests/installer-migrations/001-legacy-orphan-files) moved up one
level into installer-migrations.test.cjs; its single ../../ module require corrected to
../ so it resolves from tests/ root (verified). Host-env pre-check: no CLI-receiving host
sets a redirecting GSD_WORKSTREAM/GSD_PROJECT value.
Regenerates regression-name allowlist (222->205), ratchets file-count allowlist (verify
11->8, validate entry removed), makes 16 relocated allow-test-rule exemptions issue-ref-
compliant (ADR-456; prunes stale ids). Repoints 15 tests/ references across state-md.md
(EN + ja/ko/pt/zh). lint:ci green.

* fix(#934): reapply verifier handles missing pristine baseline post-rename
Gap 1 (verify-reapply-patches.cjs): when backup-meta.json records a
pristine_hash for a file but gsd-pristine/ has no corresponding snapshot
on disk, the verifier fell to over-broad mode and produced false
FAIL_USER_LINES_MISSING. Fix: return advisory OK_NO_BASELINE (non-blocking,
exit 0) so the verifier does not block on files it cannot reason about.
Gap 2 (new migration 004): migration 003 removed legacy get-shit-done/
runtime files but left gsd-pristine/get-shit-done/ orphan snapshots in
place. Those stale snapshots referenced get-shit-done/... key paths that
no longer match the active gsd-core/... layout. Fix: add migration
004-prune-stale-pristine-get-shit-done (NOT editing 003, preserving its
checksum — ref #670 guard) to remove all files under
gsd-pristine/get-shit-done/ as GSD-managed pristine snapshots.
Includes tests: bug-934 OK_NO_BASELINE assertions in the verifier test,
new installer-migration-prune-stale-pristine.test.cjs, updated
installer-migrations baseline-lock checksum for 004.

* fix(#934): rename migration to satisfy legacy-name guard + mark intentional path refs
Rename src/installer-migrations/004-prune-stale-pristine-get-shit-done.cts
→ 004-prune-stale-pristine-snapshots.cts so the filename no longer contains the
forbidden token. Update .gitignore and eslint.config.mjs to track the new built
path. Add gsd-allow-legacy-name markers to the remaining intentional uses of the
legacy path string in the migration body (lines 3 and 100) and in tests
(installer-migration-prune-stale-pristine.test.cjs lines 202 and 226; and the
baseline-lock key in installer-migrations.test.cjs:1469). Update the baseline
checksum for migration 2026-06-09-prune-stale-pristine-get-shit-done to reflect
the two new marker comments added to its body.

Editing the body of an already-released installer migration drifts its computed
checksum (it hashes plan.toString()). The integrity guard then hard-aborted
every prior install on upgrade with "applied migration checksum changed" — a
100% reproducible blocker (v1.3.0, all platforms).
Already-applied migrations are filtered out of `pending` and never re-run, so
a drifted checksum is functionally inert. ADR-0008 anticipates checksum-mismatch
state as something the install-state layer must handle gracefully (plan -> apply
-> recover/report), not abort on.
This supersedes the published-checksum allowlist merged in #674 (per-release
maintenance debt — every historical checksum hand-pinned, still throws for any
unregistered value) with a general, self-healing recovery:
- Replace the throwing guard with non-fatal `collectAppliedChecksumDrift`,
surfaced on `plan.checksumDrift`.
- Reconcile drifted stored checksums durably on the next state write
(`reconcileDriftedChecksums`), idempotently (no perpetual writes).
- Relocate the "shipped migration bodies are immutable" rule to a CI baseline
test that locks every shipped migration's checksum and fails on body drift —
where #615 should have been caught, instead of blocking users.
Removes #674's legacyChecksums field, per-migration checksum pins,
published-checksums.json fixture, and compat test.

* chore(#604): rename get-shit-done/ runtime directory to gsd-core/
Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the
on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary
(`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers
are unaffected.
Mechanical (bulk, ~90% of the diff):
- `git mv get-shit-done gsd-core`
- Swept path/identifier references across the repo via
`perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead
preserves the five legitimate slug variants that are NOT the directory:
get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names).
- Build/manifest wiring: package.json (bin, files, coverage globs),
tsconfig.build.json (outDir), ~86 .gitignore build-output entries,
stryker.config.mjs, scan-ignore files, install.js path strings.
- Frozen (not rewritten): CHANGELOG.md history; translated docs
(README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/).
New logic (review here):
- src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper
ADR-0008 installer migration. On upgrade it walks the legacy
`~/.claude/get-shit-done/` tree, classifies each file via the prior install
manifest, and emits remove-managed / backup-and-remove for managed files
while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked
root and symlinked entries; bounds-checks every path under configDir). The
framework rolls back on install failure. Emptied dirs may remain (framework
has no recursive dir-removal primitive) — documented.
- scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare
`get-shit-done` directory token (split token to avoid self-match; case-
insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists
CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines).
Wired into the lint-tests CI job.
- Restored scripts/lint-package-identity-drift.cjs detection regexes (the
mechanical sweep had wrongly rewritten the old-name patterns it exists to
detect) and marked them as intentional legacy references.
- TDD tests for the migration and the guard; do.md slash-command guard regex
tightened so a `/gsd-core/bin` path segment is not mistaken for a command;
changeset + docs/installer-migrations.md row added.
Breaking: the installed runtime path moves `~/.claude/get-shit-done/` ->
`~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed
files (preserving user files) on upgrade. Users with custom hooks/configs
hardcoding the old path must update them.

* fix(#604): unsweep pending changesets + allowlist injection-example docs
CI fixes for the rename PR:
- Do not sweep pending .changeset/*.md (ephemeral release-note fragments,
like CHANGELOG); reverted those body edits so 5 pre-existing malformed
fragments (missing type/pr) no longer enter the PR diff and trip docs-lint.
Allowlisted .changeset/ in the legacy-name guard accordingly.
- Allowlisted TEST-EXAMPLES.md and docs/explanation/security-model.md in
prompt-injection-scan.sh: they contain intentional injection examples /
security-model prose; the path-reference rewrites are kept.
CodeQL alerts on this PR are pre-existing (alert lines unchanged by this PR;
none in the new migration/guard) and are out of scope for the rename.
Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
* fix(#604): resolve CodeQL alerts surfaced on this PR
The rename diff touched files carrying pre-existing CodeQL findings; per the
no-pre-existing-dismissal rule, fixing every surfaced alert rather than waving
them off. All behavior-preserving:
- scripts/ci-test-scope.cjs: build the config-path match from string
.includes() instead of a RegExp over an arg-derived value (js/regex-injection).
- src/profile-output.cts: escape backslashes before pipe-escaping desc/safeName
so the table-cell escape is complete (js/incomplete-sanitization).
- tests/{bug-2643,bug-2808,docs-parity-live-registry}: two-pass HTML-comment
strip so a bare/unclosed `<!--` cannot survive (js/incomplete-multi-character-sanitization).
- tests/inline-plan-threshold: drop the no-op `\s`->`\s` identity replace,
keep the meaningful POSIX-class conversion (js/identity-replacement).
Verified: build:lib green; the touched test files + ci-test-scope + profile-output
suites pass; lint:legacy-name clean.
Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
* fix(#604): correctly resolve remaining CodeQL alerts (regex-injection + sanitization)
The prior commit's fixes for two alerts were ineffective:
- ci-test-scope.cjs js/regex-injection: the alert is the CLI-arg-derived `file`
reaching static regex `.test(file)` calls (not the config rule). Removed ALL
regex over file/t — startsWith/includes/=== string checks + an isWindowsHint
helper — so there is no regex sink for the tainted value.
- js/incomplete-multi-character-sanitization (3 test files): a single
`.replace(/<!--...-->/g,'')` can let `<!--` re-form. Replaced with a fixpoint
loop (replace until stable) plus a final bare-opener strip.
Verified: no regex over file/t remains; ci-test-scope + the 3 test suites pass;
lint:legacy-name clean.
Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
* fix(#604): make ci-test-scope + comment-strippers regex-free to clear CodeQL
CodeQL flags the regex PATTERNS syntactically (regex-injection on the
--files arg split; incomplete-multi-character-sanitization on the <!--...-->
replace), so loop fixes do not satisfy it. Made these paths regex-free:
- ci-test-scope.cjs splitFiles: char-by-char separator tokenizer (no /[,\\s]+/).
- 3 test files: indexOf/slice HTML-comment stripper (no .replace(/<!--/)).
Behavior preserved; ci-test-scope + the 3 suites pass; guard clean.

non-UTF8 bytes -> thousands of bogus blobs + "ignored null byte" warnings),
and the ~800-file rename diff is slow to scan regardless.
- scripts/base64-scan.sh: skip binary-by-content files (grep -Iq .) — they
can't carry base64-obfuscated *text* and feeding NUL bytes through the
per-line scanner is pathologically slow. collect_files already filtered
binary *extensions*; this catches binary *content* in text extensions.
- .github/workflows/security-scan.yml: raise the security job timeout 10m->30m
to accommodate very large diffs (the scan itself is unchanged).
Verified locally: scan skips the fixture, 0 "ignored null byte" warnings,
0 findings, exit 0.

* fix(#604): sweep get-shit-done refs introduced by merging next
The branch was updated with next (#614/#384/#618 etc.), which reference the
get-shit-done/ dir (still named that on next). Swept the stale references in
the merged files to gsd-core so the rename stays consistent and lint:legacy-name
passes:
- commands/gsd/discuss-phase.md (runtime-launcher shim paths)
- src/core.cts (getAgentsDir layout comments)
- tests/bug-384-agents-runtime-aware.test.cjs (require path to runtime lib)
Verified: guard 0 violations; build green.

* fix(#604): exclude gsd-core/ path segments from bug-3683 command cross-ref invariant
The #614 runtime-launcher shim added to discuss-phase.md references
`${_GSD_RUNTIME_ROOT}/gsd-core/bin/...`. bug-3683's REF_PATTERN excluded path-y
refs only via lookbehind, but `}` precedes `/gsd-core/` in the shim, so it
mis-read the directory path as a dangling `/gsd-core` command ref (same class as
the #604 bug-2954 fix). Added a trailing `(?![\w-]*\/)` so `/gsd-<x>/...` path
segments are not treated as slash-command references.
Verified locally on BOTH platforms before pushing:
- mac (node 26) full suite: 0 failures
- gsd-test-runner (linux, node22 image) full suite: 0 failures
- bug-3683 + bug-2954 pass.

* fix(#604): lazily resolve findProjectRoot in gsd-tools (harden flaky CI)
CI intermittently failed state.test's gsd-tools subprocess with
"findProjectRoot is not a function" (flip-flopping across legs; not reproducible
on mac full suite, gsd-test linux full suite, test:unit, or state.test x8).
findProjectRoot is a re-export from core.cjs (sourced from project-root.cjs);
binding it via destructure at module-load can be undefined under a load-ordering
edge. Resolve it lazily at call time via a small wrapper so the lookup happens
after core.cjs is fully initialized.
Verified green on BOTH platforms before pushing:
- mac (node 26) full suite: 0 failures
- gsd-test-runner (linux, node22) full suite: 0 failures
- state.test.cjs: 106/106; gsd-tools loads cleanly.

* fix(#604): allowlist verification-patterns.md placeholder examples in secret scan
The rename git-mv'd references/verification-patterns.md into gsd-core/, pulling
it into the secret-scan diff. It documents stub/placeholder RED-FLAG env-var
examples (illustrative Stripe test-key / database-URL / API-key placeholders) —
not real credentials. Added it to .secretscanignore with the strict annotation,
mirroring the existing gsd-core/workflows/plan-phase.md exception.
Verified locally: secret-scan-lint --strict OK; secret-scan --diff origin/next
exits 0 with 0 findings.

The windows-test-parity ratchet greps test source for fs.rmSync-without-
maxRetries (and six other Windows-portability anti-patterns), failing when an
integer offender COUNT exceeds a frozen baseline (rmSync: 95). A count ratchet
is a Goodhart metric: fixing one offender and adding another keeps the count
constant, so a new defect slips through green. Replace it — and every other
count ratchet in the repo — with a layered, masking-proof design.
Behavioral seam test
- tests/helpers-cleanup.test.cjs proves helpers.cleanup() carries the Windows
EBUSY retry budget. cleanup() delegates retries to Node's fs.rmSync via
maxRetries (it owns no loop), so the test asserts the option contract
(recursive/force/maxRetries>0/retryDelay>0) + real-FS removal + the cwd-guard,
rather than a loop that does not exist. The EBUSY risk is now tested ONCE at
the helper, not approximated textually at every call site.
Write-time ESLint rule (AST-accurate, replaces the grep)
- eslint-rules/no-raw-rmsync-in-tests.cjs (error in tests/**/*.test.cjs) bans
raw fs.rmSync, steering to cleanup(). Catches member, computed (fs['rmSync']),
destructured and aliased forms; escape hatch is inline
`// eslint-disable-next-line local/no-raw-rmsync-in-tests -- <reason>` only.
- Migrated 336 raw fs.rmSync teardown calls across ~116 test files to cleanup().
~18 genuinely load-bearing sites (mid-test SUT/fault-injection removals,
error-swallowing or name-colliding local teardown helpers) keep the raw call
with an inline eslint-disable + reason.
Shared anti-ratchet primitive
- scripts/lib/allowlist-ratchet.cjs:
- assertWithinAllowlist: fails on NOVEL ids (new offender introduced) AND on
STALE ids (a known offender was fixed but not pruned) — identity, not count,
and a ratchet DOWN toward zero.
- assertTightCeiling: a size/length budget whose ceiling must stay within a
grace band of the high-water mark, so budgets may only tighten, never creep.
Ratchets converted onto the primitive
- windows-test-parity-guard.test.cjs: rmSync rule deleted (now ESLint-enforced);
the remaining six patterns moved from integer baselines to named-set
allowlists with ratchet-down.
- scripts/lint-test-file-count.{cjs,allowlist.json}: per-module integer counts →
named filename sets (closes the swap-a-file-keep-the-count blind spot); a
module dropping under cap now FAILS to force pruning its allowlist entry.
- enh-2790 skill-count `<= 63` → named skill allowlist (ratchets toward ~58).
Size budgets hardened (tighten-only)
- agent-size / workflow-size / feat-3039 help-tiered: ceilings lowered to the
current high-water mark and an assertTightCeiling anti-creep check added per
tier. Fixed external-contract limits (description ≤100 chars, agent ≤100 KB)
are intentionally left as-is — they are not grandfathered creeping budgets.
No user-facing behavior change (tests + tooling only); no USER_FACING_PREFIXES
touched, so no changeset fragment is required.

* test(installer): add regression tests for #3670 migration lock self-deadlock
- T1: same-process PID re-entry reclamation (primary regression)
- T2: dead-PID stale lock reclamation
- T3: unlinkSync EPERM surfaces (not silently swallowed via force:true)
- T4: counter-test — normal round-trip still works
- T5: counter-test — genuinely-held live lock still errors clearly
- Update existing 'reports lock release failures' test to mock
fs.unlinkSync (not fs.rmSync) matching the fixed release path
Windows wall-clock deadlock repro depends on Docker matrix Windows runners.

* fix(installer): break migration lock self-deadlock on Windows (#3670)
Root cause: `acquireInstallMigrationLock` release closure called
`fs.rmSync(lockPath, { force: true })`. On Windows NTFS, a file
recently closed via `closeSync(fd)` may still return EPERM from
`unlink` until the OS fully releases the handle. The `force: true`
flag silently swallows EPERM, leaving the lock file on disk. The
subsequent `runInstallerMigrations` call in the same install()
invocation hits EEXIST, spins for 30 s, then throws
"installer migration lock is held".
Fix:
1. Release closure uses `fs.unlinkSync` (not rmSync+force) so
EPERM propagates via releaseError instead of being swallowed.
2. `acquireInstallMigrationLock` closes the fd before writing the
payload (path-based write), eliminating the open handle that
caused the deferred EPERM on Windows.
3. Stale-lock reclamation: on EEXIST, parse the on-disk PID and
reclaim immediately if it matches process.pid (same-process
re-entry, the primary #3670 failure mode) or if the PID is
dead (ESRCH). Live alien PIDs still trigger the 30 s timeout.
4. Error message on a genuinely-held lock now includes the holder
PID and acquiredAt timestamp for operator diagnostics.
No public API change. All callers of runInstallerMigrations are
inside installer-migrations.cjs and bin/install.js.

* chore(3670): update changeset to reference PR #3765
* fix(3670): timeout on reclaim-unlink failure to prevent spin-loop regression
When unlinkSync throws (e.g. Windows EPERM on an open handle) in the
same-PID / dead-PID reclamation path, the original code continued
unconditionally — bypassing the timeout check and reintroducing the
exact deadlock the PR is supposed to fix.
Guard the continue behind a `reclaimed` flag: only loop back to
openSync if unlink SUCCEEDED. On failure, fall through to the existing
bounded sleep + timeout, which surfaces "installer migration lock is
held" within lockTimeoutMs instead of spinning indefinitely.
Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
* test(3670): tighten T5 — assert bounded failure when reclaim unlink fails on live lock
The old T5 accepted BOTH success and throw, which allowed over-reclamation
of a genuinely un-reclaimable lock to pass undetected.
Rewrite T5 to force deterministically unreclaimable conditions:
- Pre-seed lock with process.pid (triggers isSameProcess path)
- Mock fs.unlinkSync via mock.method() to throw EPERM for the lock file
With the production fix: reclaimed=false → falls through to timeout →
throws "installer migration lock is held" within ~200ms.
Without the production fix: unlink throws but continue runs anyway →
process spins and eventually OOMs (confirmed RED: 136s runtime, V8 heap
exhaustion from infinite readLockFile + new Error() allocations).
assert.throws() now makes success a hard failure, closing the
over-reclamation gap.

* fix(3670): clean up orphan lock file when writeFileSync fails after closeSync
If closeSync(fd) succeeds (fd=null) but the subsequent writeFileSync
throws, the empty lock file was left on disk. readLockFile returns null
for an empty/invalid-JSON file, so the stale-lock reclamation path
skips it, causing the next acquire attempt to spin to timeout.
Track ownership with lockCreatedByUs flag; add a second cleanup branch
in the catch block for the fd-already-closed case.
Also fix changeset body to use the bold-prefix format required by all
other fragments in .changeset/.
