fb2d122d7f feat(#3841): assert gsd-tools identity on every state-mutating verb (#3848)
* feat(#3841): assert gsd-tools identity before any state-mutating verb

only this package publishes. The path-based branches — a project-local install,
a runtime config directory — had no such guarantee; they trusted their
configured location. This closes them.

Mechanism: once resolution finishes, and before any verb runs, the preamble
probes the tool it picked with `runtime-identity --raw` and matches the answer
with a shell `case` pattern ANCHORED to the start of the compact payload
(`{"packageName":"@opengsd/gsd-core"`). An unanchored substring match accepts
the decoy `{"packageName":"get-shit-done-cc","note":"@opengsd/gsd-core"}`, which
any colliding package could publish. The outcome is exported as the two-valued
`GSD_IDENTITY_STATUS` (`ok`/`unverified`), so the gate is asserted on a VALUE
rather than on warning prose. Rollout is warn-then-fail per the #3146 ruling:
`unverified` prints one line naming BOTH causes and continues, because
`no_identity_verb` cannot tell a foreign package from an `@opengsd/gsd-core`
older than the verb, and at rollout the old-version case is the common one.

The blocker was byte budget, not design. The preamble is inlined into 112
shipped files and several sat within single-digit bytes of frozen ceilings
(`gsd-verifier.md` 16 bytes, `gsd-executor.md` 33, `execute-phase.md` 234); a
first attempt broke five of them. What made room was collapsing the resolver's
twenty near-identical `elif [ -f … ]` arms into one candidate-list helper
(`_gsd_at`), which buys far more than the assertion costs. The preamble is now
2,624 bytes against 4,500 — a net 1,876 bytes SMALLER per inlined file, so every
capped file moved away from its ceiling rather than toward it. No cap raised, no
size-budget exception added, no override token emitted.

Resolution order, every runtime-home probe, the `unset -f gsd_run` re-source
fix, the fail-closed `exit 1`, and the `CLAUDE_ENV_FILE` persistence are all
preserved byte-for-byte in substring terms; the snippet still begins with
`_GSD_SHIM_NAME=` and still ends with `fi`, which the parity extractors anchor
on. `gsd-core/references/gsd-run-resolver.md` is re-synced byte-equal.

Also fixes two stale claims found in passing: CONTEXT.md and FEATURES.md both
described an `[ -x ]` guard as the load-bearing re-source defense. That guard
was tried and REMOVED in #3831 — it rejected the bare function name, fell
through every branch, and hit `exit 1`, which kills a sourced caller's shell.
`unset -f gsd_run` is the actual mechanism.

* fix(#3841): pair the anchor's brace by requiring a closed identity payload

The matrix went red on `tests/new-project-mvp-prompt.test.cjs` — "new-project.md
has unbalanced braces: net depth 2" — plus a knock-on report from its parent
`bug #1516` describe, which is the same failure counted once at the child and
once at the block.

Root cause: that guard (:182-189, mirroring #3784 bd53925f) walks characters and
increments on `{`, decrements on `}`, with no awareness of shell quoting. It
scans `new-project.md` PLUS every `new-project/steps/*.md`, and both
`new-project.md` and `steps/auto-mode-config.md` carry one inlined preamble copy
— hence net 2 from a snippet that was off by exactly one. The unpaired brace was
the `{` inside the single-quoted `case` pattern of the identity anchor, which is
correct shell and invisible to a text scanner.

Fix in the snippet, not the guard. The pattern now anchors at BOTH ends:
`'{"packageName":"@opengsd/gsd-core"'*'}'`. That balances 51/51 with a brace that
does real work rather than a cosmetic pair — a truncated payload whose prefix
matches now fails too, where before it verified. Safe for any future additive
field: a JSON object's own closing brace is always the last character, whatever
type the last value has, which is pinned by two negative-space tests (a nested
object and an array-valued last key must both still verify). Cost: +3 bytes,
against the 1,873 the resolver fold already gave back.

The alternative considered and rejected was dropping the literal `{` for a `?`
glob. It balances too, but weakens the anchor from "must be an opening brace" to
"must be any one character", and the anchor is the entire point.

Two guards added so this cannot recur silently:
- runtime-launcher-parity (F0) pins brace balance at the SNIPPET, so the next
edit to that pattern fails on the file it broke instead of surfacing three
files downstream in a test whose name mentions neither the launcher nor this
issue. It also asserts depth never goes negative, since a `}` preceding its
`{` nets to zero while being unbalanced at every prefix.
- runtime-identity gains behavioral truncated-payload and trailing-garbage
fixtures, so the added `}` is proven load-bearing rather than merely present.

Verified: snippet 51/51 braces; new-project combined net depth 0; the seven
other preamble-bearing files with nonzero depth are unchanged from merged next
(their own prose, not the preamble, and not in any guard's scan set); all 112
inlined copies and the resolver reference re-synced byte-equal; sync:launcher
idempotent on the second run.

63abcface9 feat(#3146): resolve gsd_run so workflows cannot reach a foreign gsd-tools (#3831)
* feat(#3146): resolve gsd_run so workflows cannot reach a foreign gsd-tools

The predecessor package get-shit-done-cc publishes a colliding gsd-tools bin whose phases.clear DELETES where this package's ARCHIVES, and both print success-shaped output against a gitignored .planning/ -- which is how #3129 cost a user 43 phase directories with no error and nothing recoverable from git.

The launcher's PATH branch now resolves gsd_run, published only by this package and self-locating via its own symlink chain to the sibling shim, instead of the colliding gsd-tools. A foreign handler becomes unreachable from PATH, and when no gsd_run is reachable the resolver fails closed rather than falling back -- that fallback was the vulnerability. This is smaller than the branch it replaces, which matters: the preamble is inlined into 113 shipped files and agents/gsd-verifier.md sits 2 bytes under a red-line size cap.

unset -f gsd_run leads the preamble so a re-source is idempotent. Without it, command -v finds the shell function, returns a bare name, and the resolver falls through to an exit 1 that kills a sourced caller's shell.

Adds gsd-tools runtime-identity, a manual diagnostic reporting this runtime's package coordinates over the baked package-identity (#498) and readHostVersion, with a strict total classifier: only a JSON object with an exact packageName verifies, since JSON.parse admits 0/"str"/[]/null/true.

An inlined identity assertion was built and reviewed first, then withdrawn -- it breaks five frozen size ceilings and no assertion fits in 2 bytes.

Closes #3146

* fix(#3146): stop sync:launcher relocating a deliberate preamble placement

Pre-existing defect, surfaced by this PR because sync is a no-op unless the snippet content actually changes. transformFile inserts the preamble into the first block that CALLS gsd_run, but gsd-core/workflows/explore.md deliberately places it in a bootstrap-only block that DEFINES gsd_run without calling it -- its own comment explains why: declining the research offer must not leave Step 5's commit call unbootstrapped. Stripping empties that block of calls, so the preamble migrated forward and broke the define-before-use invariant tests/explore-command.test.cjs pins.

Reproduced on a pristine origin/next checkout with the base snippet and base file, so this was not introduced here. The insertion target now honours a block that already carried the preamble, falling back to the first calling block for files that have none yet. Adds a behavioral regression test over a two-block fixture.

Also updates three runtime-launcher-parity tests that pinned the removed PATH fallback to gsd-tools. Their intent is preserved -- the PATH stub is renamed gsd_run so it is reachable by the new resolver, and the RUNTIME_DIR-wins test still asserts the stub is never invoked. Fixture shebangs move to an absolute /bin/sh, because the fixture PATH is deliberately restricted and #!/usr/bin/env sh could not resolve.

* docs(#3146): document the FEATURES.md section-numbering practice

The monotonically increasing section number in docs/FEATURES.md is the most frequent merge-conflict source in this repo, and it has TWO conflict cells, not one: the ### N. heading and the hand-maintained table of contents. Two PRs adding differently numbered features still collide on the TOC, so renumbering alone does not make a branch safe. This branch alone was renumbered 165 -> 166 -> 167 -> 168 across successive rebases.

Adds a CONTRIBUTING section stating the practice: allocate the number last, never pre-emptively renumber, take max+1 after a rebase and update the TOC in the same commit, and never renumber someone else's section. Fork contributors are told explicitly they may leave the number to a maintainer at merge rather than chasing the counter. Agents are told to lease the allocation and to include the file in their published touched set.

Records the durable fix as planned rather than pretending it exists: FEATURES.md should be generated from per-feature fragments the way CHANGELOG.md is generated from .changeset/, and the way tests/emitted-drift-acks/ works (#2914).

Also renumbers this branch's own section to 168, leaving 167 to the PR already in flight.

8442d984b9 fix(#3809): route runtime-loaded markdown through the gsd_run launcher (#3815)
* test(#3809): generalize dead-ref guard into a rule table (failing first)

The #2020 guard hardcoded `sdk/(src|dist|handlers)/` — the three dead paths
that had caused that storm. That proved those three paths were gone and said
nothing about the class, so #3809 reproduced the identical Windows find.exe
storm under a different token and the guard could not see it.

Replaces the single regex with a rule table over the same runtime-loaded
markdown surface, adds `commands/` to the scan set (previously uncovered),
and adds rule B: the runtime shim filename must never appear in command
position, because it is not a PATH command and an agent that meets it falls
back to locating the file.

Rule B's matcher is deliberately lenient — the launcher's own resolver
assignment, `node <path>/<shim>` calls, bare paths, and prose that names the
file all stay unflagged, each pinned by a negative-space row.

This commit is expected to FAIL: 50 offenders across 23 files remain in the
tree. The remediation lands next.

* fix(#3809): route every workflow call through the gsd_run launcher

50 places across 23 runtime-loaded workflow, agent, reference, and command
files instructed the agent to run the runtime shim by filename. That filename
is not on PATH under any name -- package.json ships gsd-core, gsd-tools,
gsd_run and gsd-mcp-server -- so the call exited 127, the file-shaped token
sent the agent looking for the file, and on Git Bash for Windows the resulting
`find /` walked the entire drive (7268 CPU-seconds in the report) until
somebody killed it by hand.

CONTEXT.md -> Runtime Launcher Module already makes gsd_run the single entry
point: "Canonical space-safe shell preamble (`gsd_run`) used by every workflow
bash block to invoke the GSD runtime CLI." These sites predate that rule --
they trace to 0e6907050 (docs(#195): migrate workflow markdown off gsd-sdk
query), which swapped one non-PATH token for another.

Two further instances of the same class surfaced during remediation and are
fixed here rather than left for later:

  - references/model-profiles.md prescribed `node <shim> effort sync` with no
path at all; node resolves a bare filename against cwd, so it fails the
    same way.
  - references/universal-anti-patterns.md rule 25 instructed every agent to
"use <shim>" when shelling out. That rule did not contain the defect, it
    prescribed it repo-wide.

Five "(or legacy <shim>)" parentheticals left dangling by the substitution are
removed; after the rewrite they offered the non-resolving form as an
alternative.

The guard from the previous commit now passes. Its node-prefix exemption was
tightened to require a path separator, which is what exposed model-profiles.

* fix(#3809): key the guard on the CLI's whole verb roster, not observed usage

Review found the first cut of rule B repeating the very mistake it exists to
prevent. Its verb set held query, commit and effort -- the verbs that happened
to appear in the tree -- so it could not see `<shim> phase add`,
`<shim> state load`, `<shim> verify ...` or twenty-odd other real single-word
subcommands. A guard that only recognises yesterday's offenders is not a guard.

The set is now the CLI's full advertised roster, unioned from the usage banner
and HOST_COMMAND_ROUTERS (which carries verification, planning, uat, stats,
todo and windows, all absent from the banner).

Widening it immediately caught a live offender the first pass had missed:
references/planning-config.md prescribed `node <shim> worktree set-baseref`
with no path. Fixed here.

Also drops the "a hyphen or a dot means subcommand" heuristic, which was
unsound for prose -- it flagged `built-in` and `v1.2`. Detection now keys
entirely on the roster, testing the first dot-segment so that phase.add and
state.patch still match while prose does not. Both false positives are pinned
as negative-space rows.

Guard verified against the pre-fix tree at origin/next: 52 offenders across 25
files, and 0 after this branch's remediation.

* fix(#3809): derive the verb roster from the router; repair launcher parity

Standards review caught the guard repeating the defect it exists to prevent.
Its verb list was a hand-copied literal -- and worse, transcribed from an
INSTALLED older binary, so it was missing 22 verbs this tree actually ships
(websearch, windows, state-snapshot, context-predicates and the dispatch-*
family among them). gsd-tools.cjs already carries three hand-maintained
rosters whose drift is a named defect pinned by the parity test in
tests/commands.test.cjs; a hand-copied fourth was that same defect wearing a
guard's clothes.

The roster is now derived from HOST_COMMAND_ROUTERS + TOP_LEVEL_USAGE, lazily
and memoised, with `query` supplemented explicitly -- it dispatches through
the routing hub ahead of the host-router table, so it appears in neither
export, yet 45 of the 50 offenders used it. A parity test pins the derivation.

Two regressions this branch introduced, both caught by the remote runner:

  - runtime-launcher-parity: rewriting a comment in gsd-research-synthesizer.md
put a `gsd_run` token at line 65 while the canonical preamble sits at 158,
breaking "exactly ONE preamble, before the first gsd_run call". The comment
is descriptive and needs no command token at all; it now names none.
  - The #2751 guard's PROSE_ALLOWLIST entry for that same line went stale once
the line stopped carrying a bare mention. Pruned, exactly as that guard's
    own stale-entry test instructs.

Also corrects git-planning-commit.md, where the first pass rewrote only the
trailing "legacy" clause and left the sentence reading backwards.

Note the #2751 guard and this one are complementary, not duplicates: its regex
requires whitespace immediately after `gsd-tools`, so it cannot match the
`.cjs` form, and this one only matches the `.cjs` form.

* fix(#2751): extend the bare-command guard to references/ and commands/

The #2751 guard has only ever scanned agents/ and gsd-core/workflows/. Two
runtime-loaded directories were never in its scan set, and 47 bare
`gsd-tools <verb>` calls had accumulated there unseen -- the same defect that
guard exists to catch, in the rooms it never entered.

  - gsd-core/references/: 37 calls, all rewritten to gsd_run. references are
fragments inlined into a parent that defines the launcher, which is why 21
of the 22 files already using gsd_run carry no local preamble.
  - commands/gsd/: 10 operative calls rewritten. The remaining 10 are
descriptive prose ("resolved inside the workflow via ...") and are
allowlisted with reasons, bringing PROSE_ALLOWLIST to 15.

commands/ also came under launcher propagation. sync-runtime-launcher.cjs
walked only WORKFLOWS_DIR and AGENTS_DIR, so every preamble under commands/
was a hand-pasted copy nothing propagated and no test checked -- graphify.md
had accumulated five. It now walks COMMANDS_DIR too, which collapses those
five to the canonical one-per-file, and runtime-launcher-parity gains a
(B-commands) arm mirroring (B-agents) exactly so the placement stays honest.

The parity arm keys on shell blocks, so commands/gsd/workstreams.md and
config.md -- which name gsd_run only in inline backtick prose -- are exempt,
as they should be. gsd_run is itself a shipped npm bin, so those inline
instructions resolve from PATH exactly as the gsd-tools form they replace did.

skills/ is deliberately NOT added to either guard's scan set: it is generated
from commands/ and pinned by lint:generated-sync, so guarding the source
guards both, and scanning the mirror would double-report every future
offender. Regenerated here.

* test(#3809): acknowledge the one emitted file this change grows

The emitted-attribution gate failed on the previous sha: gsd-research-synthesizer.md
grew 3 bytes (13847 -> 13850) with no acknowledgment. The substitution SHRANK the
other 19 emitted files, which is why the growth arm was not expected to fire at all.

The 3 bytes are unavoidable. Line 65 is a descriptive comment inside a fenced block;
naming any command there puts a gsd_run token ahead of the file's canonical preamble
at line 158, which runtime-launcher-parity's (B-agents) arm correctly rejects. So the
comment names no command and says where the config is actually loaded instead, which
reads longer than the token it replaced.

Acks only the path the gate reported, per the fragment rules.

* revert(#2751): drop the commands/ half — three contracts pin it in place

The remote runner refuted the commands/ extension outright. Reverting it and
keeping the references/ conversion, which passed.

What broke, all of it caused by bringing commands/ under launcher propagation:

  - graphify.md's five per-block preambles are LOAD-BEARING, not accumulated
    drift. tests/graphify-visualization.test.cjs extracts individual Step-3
shell chains and executes them standalone, so each fenced block needs its
own definition of gsd_run. Collapsing them to the canonical one-per-file
produced `bash: gsd_run: command not found`, exit 127, across four tests.
The "define once per file" contract holds for workflows and agents because
nothing extracts their blocks in isolation; commands/ is not like that.
  - explore.md broke "the preamble that DEFINES gsd_run must appear before the
first USE of gsd_run anywhere in the file".
  - tests/gsd-tools-path-refs.test.cjs (#1766) ASSERTS that
    commands/gsd/workstreams.md contains the literal string
    `gsd-tools query workstream.list`. Rewriting it to gsd_run contradicts a
test that pins the opposite, so the two guards disagree about that file by
    construction.

So commands/ is not a scan-set widening. It needs those contracts reconciled
first, and that is its own change. SCAN_DIRS keeps gsd-core/references/ and
drops commands/, the ten commands/ allowlist entries go with it (back to 5),
and the reasoning is recorded in the guard itself so the next person does not
rediscover it by burning a matrix run.

commands/gsd/import.md keeps its #3809 fix — that one is the .cjs form this
PR exists to remove, and it is untouched by any of the above.

Refs #2751, #3809

Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>

* revert(#3809): restore explore.md's Step 1 preamble placement

Running the launcher sync script processed workflows/ and agents/ too, not
just the commands/ directory the run was aimed at, and it MOVED
gsd-core/workflows/explore.md's preamble from Step 1 down to Step 3.

The script inserts into the first bash block that USES gsd_run. explore.md's
Step 1 block only DEFINES it, and that placement is deliberate -- the file
says so on the line above: "Placed in Step 1 rather than Step 3 so declining
the research offer cannot leave Step 5's commit call unbootstrapped."
tests/explore-command.test.cjs pins it.

explore.md carried no #3809 offender, so reverting it costs this fix nothing.
This was collateral from invoking the sync script at all, not from the
COMMANDS_DIR change, which is why the earlier commands/ revert did not catch it.

* chore(#3809): backfill PR number into changeset fragments

pr:0 -> pr:3815 for both fragments now that the PR exists.

* fix(#3809): drop the hand-rolled regex escaper CodeQL flagged

CodeQL raised js/incomplete-sanitization (HIGH) on the guard's pattern build:
`SHIM.replace(/\./g, '\\.')` escapes the dot and nothing else, so it does not
escape backslashes. It blocked PR #3815.

The repo already bans this shape -- local/no-adhoc-regex-escape exists exactly
to stop hand-rolled escapers, with the canonical one in src/pattern.cts. Rather
than reach for that helper, the pattern now carries no escaping logic at all:
SHIM is a compile-time constant whose only metacharacter is the dot, so the
regex source is spelled out literally. The generated source string is
byte-identical to what the replace() produced, verified before and after --
0 offenders on this tree, 52 against origin/next, unchanged.

A drift pin asserts SHIM_PATTERN still matches SHIM exactly, and that the dot
is escaped rather than acting as a wildcard, so the two cannot separate.

1591454357 feat(#3409): reject shell guards that cannot observe their own failure arm (#3558)
* test(#3409): failing-first regression tests for unreachable shell guard arms

Drives the three live defects fail-first, executing the shipped workflow
snippets rather than a re-typed copy:

- G1/G2 plan-phase.md Walking Skeleton gate reads `--pick summaries_total`,
a field that does not exist, so PRIOR_SUMMARIES is always "" and the gate
has never fired (#3365). G2 is the load-bearing negative-space case: it
rejects a fix that treats "no answer" as "zero" and fires unconditionally.
- G3 plan-phase.md PHASE_REQ_IDS resolves "" instead of the TBD sentinel on
  a phase with zero requirements.
- G4 complete-milestone.md's bare `cat <glob>` blocks on stdin under a
nullglob left set by an earlier block (measured hang).

Skipped on Windows for G4 only: the FIFO-blocked-stdin mechanism is POSIX
only, and a weakened assertion there would pass vacuously.

Refs #3409

* fix(#3409): make nine shell guards observe their own failure arm

`--pick` coerces a missing field to empty string and exits 0, so the
`|| echo <default>` fallback after it fires only on a verb typo, never on
the field absence it was written for. Nine sites relied on that arm.

- plan-phase.md walking-skeleton gate: `--pick summaries_total` names a
field that does not exist under any flag combination, so the gate has
never fired on any project (#3365). Repointed at the existing single
owner, `phases.list --type summaries --pick count`, which returns a real
integer in every case including a project with no `.planning` directory.
No new counter is added: a second one would duplicate the ownership
ADR-3180 Decision 1 forbids. The gate now fires only on a literal "0",
so an unanswerable query fails safe instead of entering skeleton mode.
- plan-phase.md phase_req_ids: now falls back to the documented TBD.
- The remaining seven convert to an explicit empty test.
- complete-milestone.md read all phase summaries through a bare
`cat <glob>`; under a nullglob left set by an earlier block that is zero
operands, so cat blocks on stdin. Guarded with the array shape the
#3300 fix already established in review.md.

Refs #3409

* fix(#3409): guard eleven more globs that defeat their own fallback arm

The nullglob audit this issue asks for turned up the same class in files
#3300 never touched.

- Eight bare `cat <glob>` reads (transition, complete-milestone, planner x4,
verifier, phase-researcher). With nullglob set that is zero operands, so
cat reads stdin and blocks; measured rc=137 at 3s.
- Three `ls <glob> || echo "<message>"` sites (session-report,
review-backlog and its generated skill). nullglob makes ls succeed
listing the cwd, so the message never prints and the user gets a
  directory listing instead.

Guarded with `[ -e "${_ARR[0]}" ]` rather than `[ ${#_ARR[@]} -gt 0 ]`.
The count form is correct only when nullglob is set, and six of these
seven files never set it: without it the array holds the unmatched literal
pattern, so the count is 1 and the guard passes wrongly. `-e` is correct
in both worlds. review.md keeps its count guards — that block sets
nullglob two lines above them.

skills/gsd-review-backlog regenerated from commands/, never hand-edited.

Refs #3409

* feat(#3409): add the unreachable-shell-guard drift lint

A sibling of lint-planning-prompt-drift.cjs, consuming the shared
scripts/lib/drift-scan.cjs rather than copying it, wired into lint:ci.

Both detectors are one shape — a fallback arm defeated by a legitimate
success-on-empty:

- Detector A: `--pick` and `|| echo` on one line. `--pick` is the
discriminator because "missing field renders empty at exit 0" is a
documented CLI contract, not a heuristic. A rule keyed on gsd_run
matched 111 lines, ~132 of them legitimate, and was rejected.
- Detector B: `cat <glob>` in command position, and `ls <glob>` whose
exit code feeds a real fallback or an if/while head. Informational
  `ls <glob>` whose stdout is consumed (97 sites) and `|| true` failure
suppression (~15) are not guards and never fire.

Shrink-only ratchet keyed on (file, trimmed text) with a per-pair count,
POSIX-normalized unconditionally so Windows CI cannot report everything
fresh and stale at once. Ships with a ZERO-entry baseline: every site it
can find is fixed. Exemption is the per-line `# gsd-scan-ignore: #NNN`
marker whose reason must name an issue or URL; a malformed reason reports
a distinct error rather than silently exempting. No file allowlists.

ADR-3409 records the invariant, the measurements behind both detectors,
and why the upstream `--pick` contract fix belongs to #3473.

Refs #3409

* fix(#3409): resolve review findings — typed surface, sanitized reports, tighter marker

Standards axis (blocker): the guard's tests asserted on human-readable
stdout/stderr and on free-form baseline-load prose, which CONTRIBUTING
prohibits by name. Added the typed surface it prescribes instead of
weakening the tests: a frozen REASON enum, a --json report mode,
structured loadBaseline errors, and a test locking Object.keys(REASON)
so a new reason stays three coordinated changes.

Security axis: sanitizeForReport covered every violation field but not
the baseline-load error path, which embeds raw JSON.stringify output --
that escapes nothing above 0x1f, so bidi and C1 controls reached CI logs
unfiltered. Routed through the sanitizer at the output seam.

Security axis: the scan-ignore marker accepted `#0` and a bare
`http://`. Tightened to a positive issue number and a URL with a host.
This diverges deliberately from the sibling in
tests/commit-files-pathspec.test.cjs, whose looser form was copied
verbatim; the header now records the divergence.

Security axis: G4 built its FIFO with `mktemp -u`, reserving a name
without creating it. Now created inside a `mktemp -d` directory.

Spec axis: ADR-3409 claimed a ninth site landed after the issue was
filed. git blame disproves it -- all nine predate it; the issue's hand
count missed one. Corrected. The design and test matrix still specified
B9 as a FLAG after implementation reversed it to PASS; both now record
the reversal and why.

Refs #3409

* docs(#3409): add the how-to for resolving unreachable-guard findings

Reference and Explanation are carried by ADR-3409; this is the
task-oriented quadrant CI cannot check for.

The page exists mainly for one thing the lint structurally cannot catch:
both `[ -e "${_ARR[0]}" ]` and `[ ${#_ARR[@]} -gt 0 ]` remove the glob
from the command and therefore both pass, but the count form is correct
only when nullglob is set — and nullglob is usually set in a different
block of the same file. A reference table cannot carry that; a how-to can.

Also documents the reason codes, so a reader can tell "nothing to report"
from "could not look".

No tutorial: this is a gate inside an existing CI loop, not a new entry
point a newcomer starts from.

Refs #3409

* fix(#3409): bring the touched prompt files back under their size gates

The remote run was red on 14 tests, all size/attribution, none of them
the regression suite.

- agents/gsd-planner.md was 194 chars over a 49152 cap enforced by four
separate tests, each of which says the remedy is extraction, not a bump.
It had 41 chars of headroom before this branch. Its `## Checkpoint
Types` section was an unlinked, condensed duplicate of
references/checkpoints.md, which already carries all three types and
their XML shapes; the section now points there and keeps the three
names and percentages inline. Net -969, margin 1010.
- gsd-core/workflows/execute-phase.md sat 2 chars under a comfortable
margin assertion. Dropped the AUTO_MODE default: the `|| echo "false"`
it replaced was unreachable, so the value was already sometimes empty
on next, and its only consumer compares against `true`. Net -16.
Left plan-phase.md's AUTO_CHAIN default alone -- that file names an
explicit `false` branch, so empty would match neither branch.
- Acknowledged the seven prompt files that genuinely grew, one specific
reason each. Five of those paths were already claimed by spent
fragments identical to next, which blocks a second source naming the
same path; removed just the colliding key from each, deleting the two
  that this emptied.

Refs #3409

* test(#3409): extract the whole PHASE_REQ_IDS block, not just its first line

G3 failed on the remote runner with '' !== 'TBD'. The test was wrong, not
the workflow.

The shipped contract is now two consecutive lines -- the capture and the
`${PHASE_REQ_IDS:-TBD}` default -- but the helper's `^PREFIX=.*$` regex
returns only the first match, so the test executed half the contract and
correctly observed the empty string. Renamed to extractAssignmentBlockFor
and taught it to consume the contiguous run of lines sharing the prefix.

The assertion is untouched: TBD is the right expectation, and weakening
it to accept the empty string would have reinstated exactly the class
this suite exists to catch -- a check that cannot observe the thing it
is checking.

extractFencedBashAfterAnchor is unaffected: it is fence-delimited rather
than line-anchored, so G1/G2/G4 still capture their full blocks.

Refs #3409

* chore(#3409): drop a spent ack fragment that collided on complete-milestone.md

#3458 landed on next while this branch was in flight and its fragment
claims complete-milestone.md, which this branch also grows. Two ack
sources may never name the same path.

Its entry is spent: the +9163 it explains is already absorbed at base, so
it can no longer clear anything, and the checker's own guidance for spent
entries is to delete them. Removing the key emptied the fragment, so the
file goes too -- an empty one signals nothing.

Refs #3409

* chore(#3409): backfill changeset pr number 3558

* test(#3409): hoist a regex subject out of exec() to clear the injection scan

CI's prompt-injection scan flagged `MARKER_RE.exec('# gsd-scan-ignore: ...')`.
The pattern `exec[[:space:]]*\(["']` is receiver-blind on purpose, so it
catches `require('child_process').exec('...')` -- and the scanner's own
header records that RegExp.prototype.exec is collateral, to be handled by
its allowlist.

Allowlisting the file would blind it to the real exec vector permanently,
so the subject is hoisted into a const instead: same assertion, scanner
left at full strength, no security surface widened.

abf3cf7c25 fix(#3458): scan archived milestone phases, and make [A] Acknowledge actually suppress (#3555)
* fix(#3458): scan archived milestone phases in the four audit-open scanners

`query audit-open` resolved exactly one phase root, `.planning/phases/`. When a
milestone closes its phase directories move to
`.planning/milestones/v<X.Y>-phases/`, so an item still unresolved at that
moment — the `[R]/[A]/[C]` prompt accepts "accept" and "carry forward", not only
"resolve" — became invisible to the v1.1 pre-close audit and every audit after
it. The window in which an unresolved item is visible to this gate was exactly
one milestone wide, and nothing announced when it closed.

Reproduced before fixing, with byte-identical artifacts in the two layouts and
the active layout as the control:

  active → has_open_items=true deferred=1 uat_gaps=1 total=2
  archived → has_open_items=false deferred=0 uat_gaps=0 total=0

`scanDeferredItems`' own doc comment names this as the thing it was built to
prevent — "phase directories archive to `milestones/vX.Y-phases/` (#1871) and
the entry leaves the live tree having never been triaged" — while the
implementation eleven lines below cannot read that path. It catches an entry at
its own milestone close and goes blind at precisely the transition the comment
describes.

This is not cosmetic under-reporting. `auditOpenArtifacts` sums all nine
category counts into `counts.total` and returns `has_open_items: counts.total >
0`, so four blind scanners can flip the gate's headline boolean and let
`/gsd-complete-milestone` assert a clean close it never verified. In a
fully-archived project `.planning/phases/` may not exist at all, and the
scanners' `if (!fs.existsSync(phasesDir)) return []` produced a value
indistinguishable from "nothing is open".

## One enumeration, not four

The four scanners each hand-rolled the same active-only walk. They now share
`listAuditPhaseTargets(planDir, cwd)`, which yields both roots — the shape of
fix epic #3473's B2 asks for, and the reason the fix is one seam rather than
four edits.

Three properties are load-bearing:

  * the ACTIVE enumeration is unchanged — still a raw `readdirSync`, NOT
`listMilestonePhaseDirs`. These scanners are deliberately not
milestone-filtered today, and switching would silently add window and
sentinel filtering: a behavior change belonging to #3372, not here.
  * a missing or unreadable active root skips that half instead of returning
early. That early return WAS the bug in a fully-archived project.
  * archived dirs are deliberately NOT milestone-filtered, per the comment
`src/uat.cts` already carries: archived phases belong to past milestones by
definition, so applying the current-milestone filter discards every one and
    silently reinstates this bug.

Each item now carries `archived_milestone` when it comes from a closed
milestone, matching how the sibling module already labels archived results —
without it an operator triaging `[R]/[A]/[C]` cannot tell a live item from one
carried over. Additive: no existing test or doc asserted an exact key set.

`scripts/lint-phase-enumeration-drift.cjs`'s exemption list for this file drops
from the four scanner names to the single helper, since that is now the only
place the enumeration lives.

## Tests

Written failing-first and confirmed red for the right reason before the fix, all
four driven through the real `audit-open` CLI rather than private functions:
archived-only (was 0/0/0/0 with `has_open_items=false`, now 1/1/1/1 true),
mixed active+archived (was 1/1/1/1 — the archived half dropped — now 2/2/2/2),
active-only unchanged, and an all-resolved archived phase contributing 0.

That last one passed vacuously before the fix, because the archived path was not
reached at all; it was re-verified as genuinely discriminating afterward by
flipping one archived item to unresolved and watching the count rise.

* fix(#3458): restore the scan_error sentinel and show archive provenance

Adversarial review found one BLOCKER that the previous revision introduced,
which a green remote-runner suite did not catch because nothing in the tree
asserts `scan_error` at all.

## The regression

Consolidating four hand-rolled walks into `listAuditPhaseTargets` swallowed the
active-root `readdirSync` throw in a bare `catch {}`. Pre-fix each scanner
returned `[{scan_error: true, …}]`; after, each returned `[]`. Measured with
`.planning/phases` created as a FILE (so `existsSync` passes and `readdirSync`
throws ENOTDIR):

  before this fix: uat_gaps/verification_gaps/context_questions/deferred_items
                   each `[{"scan_error":true,…}]`
  the regression: each `[]`

`complete-milestone.md` re-runs `audit-open --json` and reads those counts, so a
machine consumer could no longer tell "I/O failed" from "verified clean" — the
exact conflation this issue exists to remove, reintroduced on the failure path.
`listAuditPhaseTargets` now reports `activeUnreadable` and each scanner pushes
the sentinel shape recovered verbatim from `origin/next`, not reinvented.

The docstring claiming the active enumeration was "UNCHANGED" was false while
that sentinel was missing, and is corrected to state what is actually preserved.

An unreadable ARCHIVED root deliberately gets NO sentinel: there was no archived
read before, so there is no consumer contract to preserve, and adding one would
conflate the ordinary "no milestones archived yet" state with a real I/O failure.

## The operator could not see the archive

`formatAuditReport` is the surface the gate actually shows a human —
`complete-milestone.md` runs it without `--json` — and it never rendered
`archived_milestone`. With `01-alpha` in both roots the identical line printed
twice with nothing to tell them apart, and `[R] Resolve` sends the operator to
`.planning/phases/01-alpha/` where the archived one does not exist. Phase
numbering restarts at `01` after each archive, so that collision is the common
case, not an edge case. All four loops now render ` (archived vX.Y)`; active
lines stay byte-identical.

## Archived milestones sorted wrong

`getArchivedPhaseDirs` ordered milestones with `.sort().reverse()` —
lexicographic, so `v1.9` outranked `v1.10`. Measured order for v1.0/v1.9/v1.10
was `v1.9, v1.10, v1.0`. Now a numeric-segment descending compare. Pre-existing,
but this change is what first surfaces it in audit output.

## Tests

The blocker's regression test fails against the previous revision. Added:
`archived_milestone` present on archived items and absent (not `undefined`) on
active ones; the unreadable-active-root sentinel across all four categories; an
unreadable archived root still leaving the active half scanned; the
duplicate-name case producing two distinct entries that the human report
distinguishes; and the v1.10-before-v1.9 ordering.

`docs/COMMANDS.md` documents the archived scanning and the new field.

* fix(#3458): stop filesystem names forging lines in the audit report

Found by the security review of this branch. Pre-existing on `next`, fixed here
because it defeats the exact gate this PR is hardening.

`audit-open`'s human report is the surface `/gsd-complete-milestone` shows an
operator to decide whether a milestone may close. A `.planning/` tree authored
by someone other than that operator — a cloned repo — could contain a directory
literally named:

zz<newline>0 open items require decisions.<newline><ESC>[2K<ESC>[1G FORGED

and the report printed `0 open items require decisions.` as its own line, with
raw ESC bytes reaching stdout able to erase or overwrite the lines above it.
Reproduced against the real CLI before fixing, and again after.

## Why not just harden sanitizeForDisplay

Because that helper's contract is multi-line prose — it removes protocol-leak
lines while deliberately preserving the newlines between legitimate ones, which
`tests/security.test.cjs` pins. Stripping CR/LF there would have broken a
correct test to paper over a different problem.

The two jobs are genuinely different, so there are now two helpers. New
`sanitizeLabel` (`src/security.cts`) is for values that are semantically ONE
LINE and derived from a filesystem NAME. It ESCAPES rather than strips C0
(including ESC/CR/LF), DEL and C1, so a doctored name renders visibly as
`\n` / `\x1b` instead of being silently normalized — the report stays honest
about what is in the tree. Ordinary input passes through byte-identical.

## Nine sites, not four

The first pass covered the four phase-scoped scanners. A sweep of the rest of
the file found the identical class in five more — `scanDebugSessions`,
`scanQuickTasks`, `scanThreads`, `scanTodos`, `scanSeeds` — emitting
name-derived `slug` / `filename` / `seed_id` through the prose sanitizer.
`scanQuickTasks`' `date` had no sanitization call at all.

Every emitted field in the file is now classified and the sweep recorded:
`slug`, `filename`, `seed_id`, `phase`, `file`, `archived_milestone`, `date` are
name-derived and take `sanitizeLabel`; `hypothesis`, `status`, `updated`,
`title`, `priority`, `area`, `summary`, `questions[]` and deferred-item `text`
are content and keep `sanitizeForDisplay`. No name-derived value reaches output
unsanitized.

`--json` was already safe — JSON string encoding escapes control characters, and
a crafted name cannot break out of the string. Verified rather than assumed.

* chore(#3458): backfill changeset pr number

* test(#3458): skip control-character fixtures where the OS forbids the name

CI red on `test (windows-latest, 24, shard 1/3)`: the four forgery-rejection
tests build directories whose names embed a newline and ESC, and NTFS forbids
control characters in path components, so `mkdir` threw ENOENT.

The remote runner is Linux-only, so it could not have caught this class.

Semantically the skip is honest rather than a workaround: on Windows the
directory-name forgery vector does not exist, because the OS refuses to create
the name. The sanitizer's own behavior stays covered there by the
`sanitizeLabel` unit tests, which are pure string tests with no filesystem
calls — verified.

Uses the repo's established capability-probe convention
(`tests/adr-index-gate.test.cjs`'s `trySymlink`), which `t.skip()`s on the real
errno rather than branching on `process.platform`, and whose comment gives the
reason: a bare `return` "would silently report a PASS ... and hide the gap this
guard exists to close". A skipped test is visibly skipped.

Swept every test added on this branch for names Windows would reject or POSIX
path assumptions; these four were the only ones.

* feat(#3458): make [A] Acknowledge actually suppress, without overwriting a verdict

Making archived phases visible exposed the other half of the problem: an item
unresolved at a milestone close now resurfaces at every later close forever,
because `[A] Acknowledge` wrote a prose block to STATE.md that
`auditOpenArtifacts` never reads. `verified_closeout` became unreachable and the
gate degraded to a mandatory `[A]` every time.

## The prompt does not change

`[A] Acknowledge all` already promises "document as deferred and proceed with
close". It documented but never deferred. This makes `[A]` do what it says.
`[R]` and `[C]` stay abort paths. No "carry forward" option is invented — an
item that is not acknowledged simply keeps surfacing, which is the default.

## The marker lives inside the artifact

Not a ledger. The audit mints no ids and has no stable identity — `phase` is a
token that collides across directories, `file` for deferred items is a constant,
and identity otherwise degrades to the item's own prose after a lossy sanitizer.
Any ledger must re-derive that key every close, so a reworded item silently
un-suppresses or, worse, mis-suppresses a different one. Storing the
acknowledgment next to the thing it suppresses makes that class of bug
structurally impossible, and it is the pattern `src/uat.cts` already argues for
with `deferred-items.md`'s in-place `status: resolved`.

## The marker is verdict-preserving and self-invalidating

`status:` is never overwritten — writing `resolved` into an unresolved UAT would
be a lie in the artifact of record, and the disclosure has to be additive.

    audit_acknowledged:
      milestone: v1.0
      at: 2026-08-15
status: gaps_found # snapshot of what was true when acknowledged

Suppression applies ONLY while the snapshot still matches reality: `status` for
seven categories, `question_count` for context questions, and for deferred items
a new per-entry `status: acknowledged` distinct from `resolved`, which keeps
meaning "actually fixed". Change the artifact and the acknowledgment stops
applying, so the item comes back on its own.

That is what makes re-opening answer itself with no extra state, and it fails in
the safe direction: a stale acknowledgment can never hide a NEW problem. A
malformed marker is treated as absent — a bad marker must never silence an item.

The check is ONE shared `isAuditItemAcknowledged`, not nine copies. This file has
already been through that defect family twice in this PR.

## Observable, not silent

`audit-open --json` now reports an `acknowledged` count beside `counts`, so a
reviewer can tell a close that is clean because things were fixed from one that
is clean because things were silenced.

## Writer

New `audit-open acknowledge` verb snapshots current state itself, so the marker
is never hand-authored from workflow prose — the gap that left the STATE.md
block with no writer, no schema and two conflicting formats. Writes route
through the existing path-confinement seam.

## Two deliberate limits, failing closed

Heading-delimited deferred entries (#3457) are REFUSED with
`unsupported_heading_shape` rather than edited, because mapping a heading entry
back to its exact source span is not safely derivable when headless and heading
entries interleave in one file. A loud refusal beats a mis-targeted write.

A quick task with no summary gets one created to carry the marker, since there
is otherwise nowhere to put it.

## Tests

Self-invalidation is the important one and is covered per category: acknowledge,
then change the status or question count, and the item resurfaces. Also
malformed markers not suppressing, `status:` byte-unchanged after acknowledging,
the writer refusing a path outside the project, and the four original #3458
scenarios unchanged.

Closes #3458

Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>

* feat(#3458): wire [A] to the acknowledge verb and converge the disclosure table

Consumer side of the suppression seam.

## The workflow stops hand-authoring the mechanism

`[A]` now calls `audit-open acknowledge` once per open item, then writes the
STATE.md `## Deferred Items` table as before. The table stays as a
human-readable disclosure; it is no longer the mechanism. That closes the gap
where the block had no writer, no schema and no reader — the marker is now
written by the tool, which snapshots current state itself.

The `[R]` / `[A]` / `[C]` prompt is unchanged, `[C]` still means "Cancel — exit
without closing", and no carry-forward option is invented.

The all-clear branch now distinguishes a close that is clean because items were
FIXED from one that is clean because they were ACKNOWLEDGED, using the
`acknowledged.total` count, and carries that into the MILESTONES.md disclosure
line beside the existing override count. A clean close that was bought with
acknowledgments should say so.

## Format drift resolved

Two incompatible `## Deferred Items` shapes shipped simultaneously — 3 columns
in the workflow, 4 in the template, with different body lines. Converged on one
5-column shape carrying the source Milestone, since archived items now appear
and the archived-milestone disambiguator was previously discarded at write time.
The workflow enumerates the categories instead of trailing off in `...`.

## Ack fragment bookkeeping

`complete-milestone.md` grows 6,764 bytes (31,228 → 37,992; cap 61,440), covered
by a new `tests/emitted-drift-acks/3458-*.json`.

`2962-zsh-nomatch-for-glob-portability.json`'s `complete-milestone.md` entry is
REMOVED — the no-duplicate-path rule hard-blocks two sources naming one path.
That entry is spent: the nullglob shim it acknowledges is present in both
`origin/next` and the CI emitted baseline `fd2b97a5`, so its ripple is already
absorbed and it can never clear anything again — verified directly, not assumed,
and the gate's own message directs deleting spent entries. Its other three files'
entries are untouched.

`scripts/sync-runtime-launcher.cjs` wanted to rewrite `explore.md` as well —
pre-existing drift unrelated to this change, reverted. `complete-milestone.md`
still carries exactly one canonical preamble.

Docs cover the verb's real flag surface, the marker's verdict-preserving and
self-invalidating behavior, and the new `acknowledged` count. A second `Added`
changeset covers the verb, since the existing `Fixed` fragment describes only
the archived-phase scanning.

Closes #3458

* fix(#3458): close three blockers in the acknowledgment seam

Adversarial review of the seam. Three BLOCKERs, one of which disproves a safety
claim I published in the PR body, the changeset and the docs.

## The claim was false; the code is fixed rather than the claim softened

I wrote that "a stale acknowledgment can never hide a NEW problem". It could.
`context_questions` snapshotted only the question COUNT, so replacing two
acknowledged questions with two brand-new blockers kept the item suppressed.
`uat_gaps` snapshotted only `status`, so adding five more pending scenarios
(`open_scenario_count` 1→6) kept it suppressed.

The snapshot now identifies CONTENT, not size: a digest of the whole question
set, and a status + open-scenario-count composite. Any edit invalidates. The
other seven categories were checked and their single tracked dimension is
already the whole story. Both disproofs now resurface the item.

## Writing to the wrong line, and reporting success

`acknowledgeDeferredItem` built an unanchored regex and exec'd it over the whole
file while match-selection and the ambiguity guard ran over the section body
only, so the write landed at the first match ANYWHERE. A file with `# Notes`
holding `- Fix the parser` above a `## Deferred Items` section holding the same
bullet: the CLI exited 0 saying `acknowledged: true`, injected `status:
acknowledged` under `# Notes`, and re-audit still reported the entry open. It
corrupted unrelated content, suppressed nothing, and claimed success — and since
`--file` is unconstrained the same path could inject into a UAT or VERIFICATION
body.

Matching is now anchored to the selected section, and the matched span is
re-verified against the selected entry before any write; a mismatch refuses with
`match_verification_failed` rather than writing.

## Acknowledging todos hid the ones never shown

`scanTodos` capped at five files and then checked acknowledgment. With seven
todos, acknowledging the five that were LISTED drove `todos: 0`,
`has_open_items: false`, and items six and seven never appeared in any later
scan. The workflow's own "repeat until no todos items" remedy terminates after
one pass. Pre-feature this was unreachable because the count was pinned at five.

That is silent over-suppression — the exact direction this PR exists to remove.
Acknowledged items are now filtered BEFORE the display cap, so unacknowledged
todos beyond it still drive the count.

## The [A] branch could not fail closed

Every acknowledge call sat in a `cmd | while read` pipeline with no status
accumulation, so any refusal was discarded and the close proceeded as
`override_closeout`. Separately, `io.output` swaps payloads over 50000 chars for
an `@file:<path>` sentinel — every `jq` would then fail, every loop body run
zero times, nothing be suppressed, and the close happen anyway.

Both closed: failures accumulate across all invocations and halt before close,
and the sentinel is dereferenced using the same pattern `verify_readiness`
already uses for `INIT_MANAGER`. Quoting was verified sound by the review and is
left alone.

## Also

Suppression is now visible in the human report, not only `--json` — the
"clean because fixed vs clean because silenced" distinction was promised for the
surface an operator actually reads.

The CRLF-preservation branches in the writer were dead: every `.md` write goes
through `_normalizeMd`, which normalizes line endings and blank lines whatever
the writer does. Deleted and documented rather than left as code that cannot run.

## Why these shipped

The review named it exactly: there was no coverage for
`unsupported_heading_shape`, `ambiguous`, `not_found`, duplicate-text
mis-targeting, todos beyond the cap, or CRLF. All are now tested, alongside both
snapshot disproofs and the mixed-section fixture.

* test(#3458): align the items-open footer wording with its assertion

Remote runner red on one test: the items-open footer must match
`/previously acknowledged item/i`.

The disclosure was NOT missing — the items-open branch already printed
"N additional items previously acknowledged and still suppressed." The word
order simply did not match the regex the test in the same change asserts. A
wording mismatch between my own test and my own implementation, not a behavior
gap.

Reworded to "N previously acknowledged items also suppressed above the M open
items", which satisfies the assertion and states the relationship between the
two counts more plainly than the original did.

Swept `formatAuditReport` for other branches that could skip the tally: the only
early return is the all-clear path, which already discloses it. `scan_error`
sentinels are filtered per category and excluded from `counts.total`, so an
all-error project falls through to that same branch. No inconsistency remains.

* fix(#3458): splice by carried span, digest the untruncated question set

Security review of the writer. Both findings are the same shape, and both are
cases where an earlier fix of mine was incomplete in the same direction: a value
derived for DISPLAY was reused for an IDENTITY or LOCATION decision.

## Writing to the wrong entry, again

The previous fix anchored matching to the `## Deferred Items` SECTION but still
re-found the entry inside it with an unanchored regex, so the write landed at
the first SUBSTRING occurrence rather than the entry's own span. The
`match_verification_failed` guard could not catch it, because the mis-targeted
span is byte-identical to the target.

Probe-confirmed, in a cloned repo's own artifact:

    - CRITICAL unfixed auth bypass
      see also: - minor typo
    - minor typo

Acknowledging "minor typo" appended `status: acknowledged` into the CRITICAL
entry, suppressing it at every future close, while the typo stayed open — exit
0, `"acknowledged": true`. A variant where the target text appears inside
unrelated prose split that line mid-sentence, acknowledged nothing, and still
exited 0, so the workflow's `ACK_FAILURES` halt never fired.

Fixed structurally rather than with a better regex: `splitGapsEntriesWithSpans`
carries each entry's own character span out of the splitter, and the write
splices by that recorded span. The location is already known at selection time —
re-deriving it by searching was the entire defect class. Added as a sibling so
`splitGapsEntries`' three existing callers are untouched. With index-splicing,
`match_verification_failed` becomes a genuine independent cross-check instead of
a guard that could never fire.

## The digest was blind past the third question

`deriveOpenQuestions` truncated to three questions, and clamped each to 200
chars, BEFORE the digest hashed it — so the snapshot could not see the fourth
and later. Ship three innocuous questions, acknowledge, then add real blockers,
and they are permanently invisible: measured `open=0, acknowledged=1`, report
"All artifact types clear."

That is the same self-invalidation property this digest was added to guarantee
one revision ago. The digest now covers the untruncated list; truncation is
display-only.

Found while fixing it: the previous digest joined on a literal raw NUL byte
embedded in the source — collisions are constructible, and reachable through
attacker-controlled YAML `\x00` escapes. Verified both ways. Replaced with a
length-prefixed encoding so no two question sets can collide by concatenation.

## Sweep

Because this is the third incomplete fix on this seam, every identity and
location derivation was swept for the display-vs-identity confusion: uat_gaps
uses status plus a full-content count, the other seven categories use a scalar
status or presence, the deferred `--text` identity is never truncated, and all
five flat categories resolve their file by path rather than by content search.
No further instances.

Closes #3458

* test(#3458): correct two assertions that over-reached the measured behavior

Remote runner red on two of the F1 tests. The source is correct — reproduced
both fixtures against the built CLI — and both failures were bugs in the
assertions I wrote. `src/` is untouched by this commit.

The first is worth recording. It computed the CRITICAL entry's block as

    content.slice(content.indexOf('- CRITICAL'), content.indexOf('- minor typo'))

and `indexOf` found the FIRST SUBSTRING occurrence, which lives inside that
entry's own continuation line ` see also: - minor typo`. The block was
truncated mid-line, so the assertion could never match. The test committed the
exact first-substring-match mistake it exists to catch, one revision after that
mistake was fixed in the source.

The second asserted `deferred_items === 0` after acknowledging the typo entry,
but the decoy `- Note: reference - minor typo elsewhere, ignore` is itself an
open entry and was never acknowledged, so the correct count is 1. It now also
asserts WHICH item remains open — that is what actually proves the right entry
was suppressed, and the original assertion would have passed even if both had
been silenced.

Both now derive their expectations from measured CLI output. A comment records
that the write seam normalizes markdown (`_normalizeMd` inserts a blank line
before a list item following a non-list line) so the inserted line is not later
mistaken for a regression; that is repo-wide behavior for every `.md` write
through the single write projection, not something this change should diverge
from.

Root cause of both: the previous two dispatches verified behavior with direct
CLI probes but never executed the test file, so assertions could over-reach what
had actually been measured. Every other assertion added in those two commits has
since been re-derived from real output; no further mismatches.

589a9b29b0 fix(#2962): enable nullglob in for-glob shell blocks for zsh portability (#3087)
* fix(#2962): enable nullglob in for-glob shell blocks for zsh portability

Workflow shell blocks are fenced bash but execute in the user's login shell
(zsh on macOS). zsh's nomatch default aborts the WHOLE block on an unmatched
glob in a for-list (not just skipping the command), silently bypassing every
statement after it — including the verify-phase decision-coverage gate,
whose optional *-CONTEXT.md lookup used the unsafe for-list form so the
DECISION_RESULT= assignment on the next line never ran under zsh.

Fix: prepend a portable nullglob shim to every bash block containing a
for-glob loop:
  shopt -s nullglob 2>/dev/null; setopt NULL_GLOB 2>/dev/null
Each command no-ops (stderr suppressed) in the shell that doesn't recognize
it; the matching shell enables nullglob so an unmatched glob expands to
nothing and the loop body is skipped cleanly. Verified locally: both zsh and
bash now reach end-of-block (rc=0) on a no-match glob; bash matched-case
behavior unchanged.

14 blocks across 7 files: verify-phase.md (4, incl. the decision-coverage
gate), review.md, execute-phase.md, resume-project.md, complete-milestone.md,
audit-milestone.md, gsd-integration-checker.md, gsd-plan-checker.md (3).
Closes the zsh bypass of the #2770 fix.

ff4a57b78c chore(#1671): migrate the remaining 13 LARGE/XL workflows to the fragment model — Phase 6.3 (#3030)
* chore(#2994): fragmentize progress.md forensic audit onto the fragment model

Extract the --forensic-gated forensic_audit step to
workflows/progress/steps/forensic-audit.md behind a section marker, and
repair progress.md's init line to forward --forensic so the atom is
actually true in production rather than only under direct CLI tests.

progress.md shrinks 32630 -> 27207 bytes.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* chore(#2994): fragmentize the four manifest-wired workflows

new-project, quick, new-milestone and progress each already had a
dedicated cmdInit* entry point but zero marked sections. Extract nine
gated bodies to workflows/<wf>/steps/ behind section markers and repair
each init line to forward its flags.

Fold --full into the discuss/research/validate facts inside cmdInitQuick
so the when= grammar never sees an OR, per the chunked-mode precedent.

Fixes found while working, per the no-defer rule:
- cmdInitProgress passed no phase info to buildSectionManifestField, so
state:phase-mvp-mode was permanently false — an atom in the vocabulary
whose fact could never be computed.
- the quick init router folded flag tokens into the free-text
description, which the new forwarding would have corrupted.
- a #2508 dispatch note was nested inside quick.md's Agent(prompt=)
fence, leaking orchestrator guidance into the subagent prompt.
- progress.md had a 3-vs-4 backtick outer-fence imbalance.

* chore(#2994): fragmentize verify-work.md and admit state:ui-phase-active

Wire cmdInitVerifyWork to buildSectionManifestField — it was a dedicated
entry point that never emitted a manifest — and mark two sections.

state:ui-phase-active folds (plan:pre hooks include an active ui step) OR
(the phase dir holds a *-UI-SPEC.md) into one boolean in init.cts, so the
grammar still sees a single operator-free atom. The inner Playwright-MCP
check stays as prose inside the fragment: it is live session state and no
init seam can precompute it.

The MVP false-branch note is a real fallback, not redundant prose, so it
sits outside the marker — gating it away would delete the text needed
precisely when MVP mode is off.

* test(#2994): follow moved workflow content in drift guards

Retarget every guard that asserted on content this branch moved into
workflows/<wf>/steps/, mirroring 815b3d897. Each retargeted assertion was
verified to still fail when its step file is blanked, so none was
weakened into vacuity.

Three assertions in verify-mvp-uat were genuinely red. Three more were
worse than red — passing for the wrong reason:
- quick-commit-boundary and worktree-cleanup anchored on indexOf('Step
5.6'), which matched a later cross-reference and sliced 16069 chars
that coincidentally held the asserted substrings. Replaced with an
expandWorkflowSections helper that splices step content back in place.
- phase6-review-capabilities lost its end boundary and widened to EOF.
- playwright-ui-verify matched 'UI' in an unrelated bullet and 'fall
back' in a subagent-dispatch line after the real content moved.

* chore(#2994): fragmentize code-review and complete-milestone, admit three atoms

Add dedicated cmdInitCodeReview and cmdInitCompleteMilestone entry points
alongside the shared generic ones rather than modifying them — init.phase-op
and init.manager carry a CRITICAL blast radius (179 dependents, 24
processes) and stay byte-identical for their other callers.

Admit flag:--fix, state:fallow-enabled and state:git-create-tag, each with
a consuming section and a fact its own entry point computes.

Both sections had the resolver-in-body hazard: the fallow config-gate and
the git.create_tag check each sat inside the very block being gated, so
gating would have disabled the resolver that decides the gate. Both are
hoisted into init and the bodies now consume the resolved fact.

* test(#2994): retarget code-review and milestone drift guards, fix two red tests

Retarget guards that asserted on content moved into steps/, proving
non-vacuity by blanking each step file and confirming failure.

Also fixes two genuinely red tests found while working, per the no-defer
rule:
- workflow-fragments' frozen-vocabulary lock was missing
state:ui-phase-active, so commit 7ef7f8336 shipped red. Lint and build
both passed over it, which is why neither is sufficient verification.
- code-review's quick.md capability-hook assertion carried a stale
  delimiter after the 18ff35d20 extraction.

* chore(#2994): fragmentize autonomous.md and admit state:plan-strategy-converge

Five sections share one atom, the pattern plan-phase already uses for
flag:--research-phase. The atom folds --converge OR --cross-ai into a
single boolean in cmdInitAutonomous so the grammar stays operator-free.

cmdInitAutonomous is additive; init.milestone-op, init.manager and
init.phase-op are untouched and still consumed. The $PLAN_STRATEGY bash
resolver is deliberately retained — ungated local-planning bullets still
read it, so the init-side fact supplements it rather than replacing it.

converge-fail-fast required splitting one bash fence so the always-run
CONVERGENCE_ARGS construction stays outside the marker. All three
flag-absent fallbacks were left outside their markers.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* chore(#2994): fragmentize review and discuss-phase-assumptions

Admit state:reviewer-instances-configured (two peripheral notes share it;
the core reviewer-lane dispatch stays unmarked — it is the workflow's
primary always-evaluated logic, not an optional branch) and
state:auto-advance-active, which folds --auto OR two config keys into one
boolean so the grammar stays operator-free.

discuss-phase-assumptions was the highest-risk edit in this PR. Its
auto_advance step is a full if/elif/else; gating it whole would have
deleted the flag-absent fallback needed exactly when --auto is off. Split
verified exact: resolvers 636-651 and the 'End here' fallback 668-669 both
stay outside the marker; only 653-667 is gated.

Adds emitted-drift acks for the two files that grew — review.md (+55 B)
and autonomous.md (+737 B from 80799211c, which had none and would have
red-gated the push.

* chore(#2994): fragmentize docs-update, update, transition and new-milestone Part A

Completes the 13-workflow rollout. Three of these had no init call at all
and gained a dedicated entry point plus their first gsd_run query line.

Admits state:is-monorepo and adds state:next-channel, state:workstream-active
and state:flat-mode. Vocabulary 26 -> 30 atoms.

Part A of new-milestone applies when NO workstream is active — the negation
of state:workstream-active. Rather than teach the grammar negation, which is
the Greenspun drift the frozen list exists to prevent, it gets a separate
positively-phrased atom whose fact is the inverse. Part B, which always runs,
stays outside the marker.

flag:--verify-only is deliberately NOT admitted: docs-update has no
contiguous purely-additive region for it, and an atom without a consuming
section is dead vocabulary. Evidence recorded in the slice report.

update.md reuses its existing resolved $GSD_TOOLS rather than prepending the
canonical preamble, which would have clobbered it.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* fix(#2994): stop automated-ui-verification re-resolving its own gate, retire dead vocabulary

Two defects the new tests caught.

The automated-ui-verification step re-ran gsd_run loop render-hooks and
recomputed UI_PHASE_ACTIVE inside a body that is only read when that fact
is already true — the circular self-disabling pattern this design forbids,
introduced by 3c654b168. cmdInitVerifyWork now exposes ui_phase_active and
the step consumes it. Its launcher preamble goes too: no gsd_run remains.
The Playwright-MCP check stays as prose — that is live session state.

Dead vocabulary predating this PR: flag:--full and state:needs-codebase-map
were admitted with a gate-1 claim that never materialized. flag:--full is
removed, redundant once quick folds it into discuss/research/validate.
state:needs-codebase-map gets the real consumer it always lacked, gating
new-project's codebase-map offer. Vocabulary 30 -> 29, and no atom is now
without a consuming section.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* test(#2994): add the atom-admission, inversion and resolver-hoist gates

The two existing parity guards prove vocabulary/predicate symmetry but
never that a fact is computed — an atom no cmdInit* assembles evaluates
false forever. These close that hole:

- per-atom satisfiability for all 29 atoms, plus an anti-vacuity assertion
so the loop cannot silently cover zero atoms
- dead-vocabulary check against the shipped manifest
- inversion guard: the flag-absent fallbacks in discuss-phase-assumptions
and verify-work must stay outside their markers
- data-driven resolver-hoist guard over the shipped manifest, so a future
extraction cannot reintroduce the circular class
- compound-fold coverage (--full, --cross-ai, --rc, config-only --auto)
- null-vs-[] degraded/computed distinction, and flag value shapes

Also repairs the frozen-vocabulary lock, which was stale and red for the
seven atoms earlier commits on this branch shipped.

* fix(#2994): dedupe the phase-fallback block and harden --ws parsing

Review findings.

MAJOR: the three new init entry points each pasted a verbatim copy of the
guardedFindPhase/guardedGetRoadmapPhase fallback, taking the repo from four
copies to seven — DEFECT.GENERATIVE-FIX. Extracted applyRoadmapFallback and
folded six of the seven; each call site keeps its own field-set via a
closure. Duplication removed rather than papered over with a parity test.
cmdInitPhaseOp stays out: its fallback omits has_reviews, so it is not a
byte-identical copy, and it is CRITICAL-radius.

LOW, pre-existing: GSD_WS captured [^[:space:]]+ and expands unquoted, so a
workstream name holding glob metacharacters would expand against the
filesystem. Narrowed to [A-Za-z0-9._-]+. The unquoted expansion is kept —
it must word-split into two args and vanish when empty.

Also restores the vocabulary ordering convention, and fixes a masked test
bug the mandated run surfaced: the flag-forwarding guard checked only the
first init line per workflow, but new-milestone has two, so a real failure
was reporting exit 0.

* fix(#2994): drop the stale new-milestone emitted-drift ack

new-milestone.md was acked for a +406 B growth measured against an
intermediate commit. Net against origin/next it SHRANK by 8 bytes, so
nothing needed the ack and it explained nothing — which the differential
attribution check reports as a stale acknowledgment, not a pass.

update.md's entry stays: it genuinely grew +703 B.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* fix(#2994): resolve the 15 failures from the full matrix run

All 15 were real and identical on both lanes.

REAL REGRESSION: autonomous.md hit 41479 chars against the #2196 guard's
40960 cap — a CHARS cap distinct from the LARGE tier byte cap, which the
five section stubs pushed it over. Extracted the 3a.5 UI Design Contract
body to references/; now 39968 chars, and the file nets -795 B vs base, so
its growth ack is deleted rather than left stale.

REAL DEFECT: docs referenced /gsd-transition, which is not a live
registered command. Reworded.

STALE FIXTURE: the emission byte-identity test hardcoded two marked
workflows; this branch legitimately marks fifteen. Fixture corrected — the
source was right.

The rest were drift guards over the eight workflows the earlier sweep did
not cover, retargeted at where the content now lives with non-vacuity
proven by blanking each step file and confirming failure. The GSD_WS
forwarding guard was checked as a possible real break and is not one: the
charclass narrowing is intact and forwarding works end to end.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* fix(#2994): drop the ack for a newly-added reference file

A new file's emitted ripple is attributable to the diff that adds it, so
the acknowledgment explained nothing and the differential check reports it
as stale. Removing the last entry removes the fragment — an empty one
signals nothing.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* fix(#2994): retarget the UI-contract guards and clear two transitive advisories

The §3a.5 extraction that brought autonomous.md under the #2196 char cap
moved its body to references/autonomous-ui-design-contract.md, so ten
guards in autonomous-ui-steps and check-ui-safety-gate were asserting it
against the host. Retargeted via a combined read, each proven non-vacuous
by blanking the reference file and confirming failure.

This class had already bitten twice on this branch because each sweep was
scoped to the workflows touched at that moment, so this one was
exhaustive: ~70 test files across all 13 workflows, zero further broken or
vacuous assertions found.

Also clears two high transitive advisories the matrix flagged on one lane
— fast-uri GHSA-7p8r-x3mc-p8w7 and three ip-address SSRF/trust-boundary
issues. Both pre-date this branch: package-lock.json was untouched until
now, so the production tree was byte-identical to the base. Lockfile-only,
package.json unchanged, verified against a real npm ci install.

c61dd49d95 enhance(#2255): blocking catastrophic-shrink guard for curated .planning/ writes (#2301)
* feat(#2255): blocking catastrophic-shrink guard for .planning writes

Adds hooks/gsd-write-guard.js, a PreToolUse hook that hard-blocks
(decision: 'block', exit 2) a whole-file Write collapsing a curated
.planning/ artifact (ROADMAP.md, .planning/milestones/*-ROADMAP.md,
STATE.md) below 40% of its on-disk line count. Files under 40 lines
are exempt; GSD_ALLOW_PLANNING_SHRINK=1 (named in the block message)
bypasses for legitimate milestone resets.

Fix 3 of #973 — the only defense independent of per-agent tool config.
Registered on the Claude plugin surface (hooks.json), settings-json
runtimes (runtime-hooks-surface.cts, self-contained pattern), Kimi
spec, and the OpenCode/Kilo plugin buses. Golden install fixtures and
INVENTORY regenerated; regression tests negative-controlled (16/16
RED with the hook absent, 16/16 GREEN with it present).

* chore(#2255): backfill changeset pr number to 2301

* enhance(#2255): address review — fail-closed reads, typed block output, registration, property test

Review fixes for trek-e's CHANGES_REQUESTED on PR #2301:

- Blocker 2: register gsd-write-guard.js in BUNDLED_GSD_HOOK_FILES
  (no-shipping-drift test).
- Blocker 3: update the always-on hook enumerations in ADR-766 and
  CONTEXT.md from six to seven.
- Major 4: fail CLOSED on non-ENOENT read errors — only a missing file
(new-file Write) passes; EACCES/EISDIR/ELOOP/etc now block, with a
typed readError field and the override still honored. Tested, with a
negative control against the pre-fix hook.
- Major 5: fast-check property test for the SHRINK_RATIO/FLOOR_LINES
budget contract (blocked ⟺ newLines < oldLines*SHRINK_RATIO above the
floor; sub-floor always exempt), boundary examples pinned.
- Major 6: block output now carries typed oldLines/newLines/
overrideEnvVar fields; tests assert on those instead of regexing the
  free-form reason string.
- Minor: CURATED_PATTERNS are case-insensitive (case-insensitive-FS
bypass on macOS/Windows); limit+1 boundary tests added for both the
  floor and the ratio.

* enhance(#2255): engage the write guard on Kimi's native payload shape

The guard shipped with Claude-vocabulary checks (tool_name 'Write',
tool_input.file_path), which #2304 showed leaves a guard dormant on
Kimi: the [[hooks]] matcher is registered pre-translated but kimi-cli
forwards its native payload verbatim — tool_name 'WriteFile' (bare or
module-qualified) and tool_input.path per its tool schemas
(src/kimi_cli/tools/file/write.py). The guard matched, saw an unknown
name, and exited 0.

Apply the same per-guard normalization PR #2326 gives the three
sibling guards (name + field mapping, inlined — hook scripts stage as
standalone files), and write the block reason to stderr as well as
stdout JSON: Kimi feeds stderr, not stdout, back to the model on
exit 2, so a stdout-only reason blocks without telling the model why
or naming the documented override.

Regression tests pipe Kimi-shaped payloads (engage, qualified-name,
stderr-reason) plus exemption pins (StrReplaceFile stays out of scope
by design; non-curated paths pass) — verified red against the pre-fix
guard, green after.

* enhance(#2255): rebase onto next; regenerate golden-parity fixtures

* enhance(#2255): wire the escape hatch into complete-milestone's reorganize step

Review Blocker 1: the guard hard-blocked /gsd:complete-milestone's ROADMAP
reorganize — the tree's only legitimate milestone reset and the exact caller
GSD_ALLOW_PLANNING_SHRINK was built for. The reorganize step now performs the
rewrite through a shell write with the hatch set on the command (a hook
inherits the runtime env, so a bare Write cannot carry a per-step override),
and a binding test derives the env var name from the guard's typed output and
asserts (a) the workflow step sets it and (b) the guard passes the identical
catastrophic payload under it — so the next complete-milestone.md edit cannot
silently re-break the wiring.

* enhance(#2255): drop dead Edit-class mapping from normalizeKimiPayload

Review Major 1: StrReplaceFile -> 'Edit' and the old_string/new_string
reconstruction were unreachable-by-effect — the guard exits 0 for any
tool_name !== 'Write', so nothing ever read the fields they set, leaving
guaranteed-surviving mutants against the Stryker bar. The map now carries
only WriteFile -> 'Write'; the StrReplaceFile exemption test message states
the fall-through it actually exercises.

* enhance(#2255): review minors — American spellings; writeSync before exit(2)

Minor 1: normalised/normalise -> American house style. Minor 2: the two
block paths wrote stdout+stderr via async pipe writes then exit(2) —
async-on-Windows, unflushed at exit; fs.writeSync(1/2, ...) makes the block
payload durable.

* enhance(#2255): assert stderr equals the typed reason, not raw prose

Minor 3: the last raw-text match in the suite pinned override-name prose on
stderr. The contract is "stderr carries the reason Kimi feeds back" — now
asserted as stderr non-empty and byte-equal to the parsed stdout.reason.

* enhance(#2255): bind the write-guard's Kimi normalization into the parity test

Review Major 2: the guard's normalizeKimiPayload is a 4th inlined copy with
nothing binding it. This extends PR #2326's kimi-guard-normalization-parity
test (same path and helpers, authored as a superset so either merge order
resolves cleanly): sibling byte-parity is existence-gated zero-or-all —
trivially green until #2326 lands, full-strength after — and the write-guard
copy is bound semantically (map is the value-inverse of convertKimiToolName;
the Kimi name for Write must map, or the guard is dormant on Kimi; the
path -> file_path half must be present). Byte-parity is deliberately not
asserted for this copy: it legitimately omits the Edit-class mapping
(Major 1 — dead code in a Write-only guard).

* enhance(#2255): refresh golden-parity fixtures for revised guard + workflow

* chore(#2255): regenerate golden fixtures after rebase onto next

The committed fixture hashes were generated against a tree predating
next's latest 11 commits, which independently modified the same
install-parity surface. Rebased onto next and regenerated with
`npm run gen:golden`.

Verified: against upstream/next the regenerated fixtures differ by
exactly this PR's own entries -- hooks/gsd-write-guard.js (new),
hooks/managed-hooks-registry.cjs, plugins/gsd-core.js, and
gsd-core/workflows/complete-milestone.md. No unrelated drift.

* fix(#2255): regenerate workflow size baseline for complete-milestone

`complete-milestone.md` grew 31071 -> 32061 (+990) when the round-2
review fix bound GSD_ALLOW_PLANNING_SHRINK=1 into the reorganize step,
but tests/workflow-size-baseline.json was never regenerated. The
per-file workflow baseline test (issue #1074) failed on
ubuntu-latest/22 and both macOS shard 1/3 jobs.

The growth is justified: it is the escape-hatch binding requested in
review round 2 (the guard must not hard-block the tree's only
legitimate milestone reset), not incidental bloat.

Regenerated via `npm run size:baseline`; the diff is exactly the one
entry.

* chore(#2255): regenerate golden fixtures and size baseline after rebase onto next

* enhance(#2255): bind the shrink escape hatch mechanically — single-use sentinel the guard consumes

Round-5 M1: the per-step `GSD_ALLOW_PLANNING_SHRINK=1 tee` prefix was inert
(no PreToolUse hook exists on Bash in this family; the write succeeded by
dodging the guard, not by the override firing) and the protection was prose.
The hatch is now a transport code consults: complete-milestone's reorganize
step arms `.planning/.gsd-allow-shrink` with the target's path, keeps the
Write tool as the sanctioned path, and the guard — at the block point only —
verifies the sentinel is fresh (15 min) and names the pending target, then
CONSUMES it and allows that one write. Path-bound + single-use + freshness
keep it from becoming a standing unlock. The env var remains as the
interactive transport, where it can actually reach the hook.

Regression tests written first (negative control: 3 failed pre-fix): the
armed-sentinel Write passes and consumes; stale does not exempt; a token for
a different file neither exempts nor is consumed; the binding test now takes
the sentinel name from the guard's typed output (overrideSentinel), asserts
the step arms it, and asserts the step no longer routes the rewrite around
Write via a shell pipe.

Also in this commit, same file:
- m2: block emission is exception-safe — emitBlock() wraps both writeSync
sites in their own try/catch that still exits 2, so an EPIPE can no longer
  convert fail-closed into the outer catch's fail-open.
- Header discloses the two reviewed design limits (cumulative sequential
shrink; lexical match vs symlinked paths) per round-5 scoping.

* docs(#2255): document the sentinel transport across guard surfaces; changeset ends with the (#2255) parenthetical (m4)

USER-GUIDE bullet, INVENTORY row (en + ja/ko/pt/zh), the
runtime-hooks-surface registration comment, and the changeset now describe
both hatches — the single-use sentinel for workflow steps and the env var
for interactive use — instead of implying a per-step env can reach a hook.
The changeset's trailing `Resolves #2255.` prose becomes the `(#2255)`
parenthetical the repo's fragments use (round-5 m4).

* chore(#2255): regenerate derived families on the rebased tree (full sweep)

Full generator sweep after rebasing onto next @ the body-parser-patched
lockfile: build, gen-inventory-manifest, gen:golden, size:baseline. Every
regen delta verified to be either a PR-owned entry (gsd-write-guard.js,
complete-milestone.md, INVENTORY/USER-GUIDE) or exact convergence to next's
committed value for entries our arbitrary-side conflict resolution had left
stale (all 18 runtime fixtures checked mechanically).

* test(#2255): use helpers.cleanup for sentinel teardown, not raw fs.rmSync

The repo's local/no-raw-rmsync-in-tests rule exists for the Windows-EBUSY
retry budget; the sentinel disarm now rides it like every other teardown.

* chore(#2255): regenerate derived families after rebase onto next

Full sweep on the rebased tree (build -> gen-inventory-manifest ->
gen:golden -> size:baseline). Every delta is either a PR-owned entry
(hooks/gsd-write-guard.js, its registration surfaces
hooks/managed-hooks-registry.cjs and the two plugin buses,
gsd-core/workflows/complete-milestone.md) or exact convergence to
next's committed value across all 18 runtime fixtures.

* chore(#2255): regenerate derived families after rebase onto next @ a5180d96

Rebase onto current `next` (a5180d96) resolved 12 conflicting
golden-install-parity fixtures; all regenerated via the full generator
sweep (build, gen:golden, size:baseline) rather than a single generator.

`lint:generated-sync` reports every generated artifact in sync. All 45
differing fixture keys and the single workflow-size-baseline entry map
to files this PR actually touches; no foreign drift.

* fix(#2255): remove the stale unguarded reorganize_roadmap step (round-8 blocker)

complete-milestone.md carried a second ROADMAP-collapsing step,
`reorganize_roadmap`, distinct from the sentinel-armed
`reorganize_roadmap_and_delete_originals` this PR wired. It is a vestige
of the pre-archive-then-reorganize design: it sits BEFORE
archive_milestone, so executing it as written would collapse ROADMAP.md
before the archive snapshots the full phase detail — and its Write is
exactly the shape gsd-write-guard hard-blocks, with no hatch armed. The
file's own success criteria describe only one reorganize outcome
(Backlog-preserving, overwrite-in-place — the later step's properties),
and archive_milestone points forward to "the reorganize step".

Removed rather than wired, per the round-8 review's confirm-and-remove
option. A new binding test asserts the sentinel-armed step is the ONLY
reorganize step in the workflow, so an unguarded collapse step cannot be
silently reintroduced (negative-controlled: fails against the pre-fix
tree). Golden-parity fixtures and the size baseline regenerate for the
shrunk file; every changed fixture key is complete-milestone.md's own.

* test(#2255): document why the read-error injection is a path collision, not an fs monkeypatch

Round-8 nit: the non-ENOENT tests inject via a directory-at-target-path
collision instead of the repo's fs-method monkeypatch pattern. That is
deliberate, not drift — runHook exercises the hook as a spawnSync child
process, so an in-process fs.readFileSync patch (the pattern the cited
siblings use on require'd, in-process code) can never reach the code
under test. Record the reasoning at the injection site.

* chore(#2255): regenerate derived families after rebase onto next @ 0d08c320

Rebase onto current next (0d08c320) for the CONFLICTING/DIRTY state. All 32
conflicts were generated artifacts (19 golden-install-parity, 12 install-tree,
workflow-size-baseline); resolved arbitrarily and regenerated via a full
generator sweep (build, gen:golden, size:baseline, gen-inventory-manifest)
rather than hand-merged. No source conflicts.

Regen diff verified against the PR's changed-file set: 7 distinct differing
keys, all PR-owned (gsd-write-guard.js, managed-hooks-registry.cjs,
plugins/gsd-core.js, complete-milestone.md, and their .kimi mirrors).
lint:generated-sync clean.

* chore(#2255): regenerate derived families after rebase onto next @ 9138271b

Conflict set was 20 paths, every one a generated artifact, zero source
conflicts — resolved arbitrarily during the replay and regenerated here,
per the maintainer's round-9 recipe (never hand-merged).

Generator sweep (not just gen:golden): npm run build, gen:golden,
size:baseline, gen-inventory-manifest, gen:registry. INVENTORY-MANIFEST
came back byte-identical, so the merged value was already correct.

Regen diff verified == PR-touched entries: every differing leaf key
attributes to a file this PR changes (complete-milestone.md, the guard,
managed-hooks-registry.cjs, both plugin buses) — derived from the
changed-file set, not a feature-name filter.

* fix(#2255): port the #2595 Kimi-payload hardening the base landed for the sibling guards

Self-found via the round-10 base sweep: a8b40fa5 (#2547/PR #2595) hardened
gsd-prompt/read/worktree-path-guard's normalizeKimiPayload after this PR was
authored, and this guard carried all three pre-fix shapes:

- `path` now wins outright over a model-supplied `file_path` — kimi-cli's
WriteFile schema carries no `file_path`, so a payload pairing a curated
  `path` with a spurious `file_path: ""` had the guard reading '' and
exiting 0 while kimi-cli wrote to `path` (one-key bypass, no crash needed).
- null/primitive payloads pass through deliberately instead of throwing into
  the fail-open catch.
- the `file_path` read is typed (`typeof === 'string'`), closing the
  crash-to-allow on `[]`/`{}` — this is what the base-added
  kimi-guard-typed-payload-reads scan flags.

Negative-controlled: the shadowing payload exits 0 against the pre-fix guard
and 2 against this one.

* fix(#2255): round 9 Major 2 — stop printing the sentinel recipe in the agent-facing denial

The block message ended by telling the blocked agent exactly how to arm the
single-use sentinel — handing the #973 actor a mechanical self-authorization
in the moment it is blocked. The recipe is gone from the prose; the
corrective advice and the env override's name stay (the latter is a #2255
acceptance criterion, and a per-step env prefix cannot reach a hook anyway),
and the typed overrideSentinel field stays for the binding tests. The hatch
remains documented in USER-GUIDE.md and complete-milestone.md, where humans
and the workflow engine read.

* fix(#2255): round 9 Minors 1-2 — realpath-resolve the target before the curated match; disclose the /i Linux cost

Minor 1: a Write to a non-curated path that symlinks into a curated file was
not matched while writeFileSync followed the link — the target is now
realpath-resolved before the curated match (ENOENT keeps the lexical
resolution so new-file Writes still pass; any other realpath error falls
through to the read, which fails closed). Negative-controlled: the symlink
payload exits 0 against the pre-fix guard, 2 against this one. Test skips on
win32, where symlink creation needs privilege.

Minor 2: the header's design-limits block now names the unconditional /i
cost on case-sensitive Linux (a genuinely distinct .planning/roadmap.md is
also treated as curated) next to the stateless limit, and drops the closed
symlink limit.

* test(#2255): round 9 Minors 3-4 — CRLF counting pin + a passing Write leaves a fresh sentinel unburned

Minor 3: countLines' split('\n') is CRLF-safe for a count (the \r rides
along), confirmed by trace in the review — this pins it against this repo's
recurring CRLF regressions, on both sides of the compare and at the 40%
boundary.

Minor 4: consumeSentinelFor runs only after the ratio check would block, so
a within-tolerance Write never burns the workflow's token — true by
construction, previously un-asserted.

* fix(#2255): round 9 Major 3 — correct the stale env-var line in archive_milestone's summary

complete-milestone.md's "After archival" bullet still said the reorganize
happens "under GSD_ALLOW_PLANNING_SHRINK=1" — the wording from the round-2
design this PR's own history rejected in round 5 (a per-step env var cannot
reach a hook; setting it in a Bash step silently does nothing). It now points
at the sentinel mechanics the reorganize step actually documents, matching
that step and USER-GUIDE.md.

* docs(#2255): round 9 Major 1 — user-facing docs state the stateless per-Write limit

The changeset and USER-GUIDE described the guard as covering "catastrophically
shrinks" with no caveat, while the stateless design was disclosed only in the
hook header — an operator reading the shipped docs would conclude iterative
erosion is covered. Both surfaces now state the per-Write comparison and the
erosion non-goal explicitly, in line with what the guard does.

* fix(#2255): round 10 Blocker 1 — arm the hatch in the roadmapper's curated writes

gsd-roadmapper Step 7 Writes both .planning/ROADMAP.md and .planning/STATE.md
wholesale, and /gsd:new-milestone spawns it against the OUTGOING milestone's
files. new-milestone's `phases.clear` archives phase DIRECTORIES, never
ROADMAP.md, so nothing compacts it first, and no ordering rule forces
/gsd:complete-milestone to run before /gsd:new-milestone.

Measured against the shipped hook at the #973 file size (292 lines): a new
4-phase roadmap lands at 18.2% and an 8-phase one at 31.8% — both blocked; only
a 12-phase replacement (45.5%) clears. So the collapse is reachable on a
first-party flow, which is the failure mode #2255 para 4 says the guard must not
have.

Arms the single-use sentinel immediately before each Write, one per target
(the token is path-bound and single-use, so one arming cannot cover both), and
gates each arming on `[ -f ]` so the /gsd:new-project path — where the guard
already exempts via ENOENT — strands no unconsumed token.

Binding test derives the sentinel name from the guard's own typed output, so a
rename on either side fails the suite instead of silently unwiring the hatch.
Negative-controlled: 2 assertions fail against the pre-fix roadmapper.

* docs(#2255): round 10 Major 2 — scope the claim to the guarantee that ships

The sentinel is a plain file, so an agent that would reason past an advisory
can arm one with a single Bash call it is already permitted to make. The
"only defense independent of per-agent tool config" framing outran that, and
the changeset was on its way into CHANGELOG.md.

Retitles the claim on all three surfaces (changeset, guard header, USER-GUIDE)
to what the guard actually delivers: it blocks accidental and single-shot
collapse and is not a defense against a determined agent; what it converts is
"ignore a sentence" into "take one deliberate, path-bound, single-use,
auditable action".

Pinned by test on the DURABLE surfaces only — the guard header and USER-GUIDE.
The changeset fragment is deliberately not pinned: it is consumed at release,
so a test reading it would start failing the moment the release lands. The
bound-statement assertion normalizes comment markers and whitespace first, so
it pins the claim rather than the paragraph's line wrapping.

Negative-controlled: both assertions fail against the pre-fix surfaces.

* test(#2255): acknowledge the roadmapper growth from the round 10 Blocker 1 wiring

The emitted-attribution gate (#2719/#2767) flags gsd-roadmapper.md growing 1130
bytes without an acknowledgment. The growth is the Blocker 1 sentinel wiring
plus the rationale a future editor needs to keep it, so it gets an ack fragment
rather than a silencing regen — the gate's own message is explicit that there is
nothing left to regenerate.

Fragment is PR-scoped (2301-…) per the gate's naming instruction, and uses the
plain-string reason form the shipped fragments use.

Verified against the TRUE upstream tip, not the fork's origin/next: a stale
origin made this same gate report unrelated phantom drift (1 emitted path + 6
grown files + 5 stale acks) that vanishes when GSD_EMITTED_BASE is pinned.

* test(#2255): renumber the roadmapper PROSE_ALLOWLIST pin after the Step 7 wiring

CI red on shard 2/3, all four platforms. The #2751 gate keys PROSE_ALLOWLIST on
{file, line}; the Blocker 1 wiring added 18 lines above the allowlisted
parenthetical in agents/gsd-roadmapper.md, moving it 624 -> 642. Both halves of
the gate then fired: the moved line reads as a new offender, and the stale
entry no longer matches anything.

Line content at 642 is byte-identical to what the entry describes — a
descriptive "e.g." naming SDK queries a user could run — so this is a
renumber, not a re-classification.

Swept the defect class rather than the instance: agents/gsd-roadmapper.md is
the only line-pinned reference to any file this round changed.

Negative-controlled: both assertions fail against the un-renumbered allowlist.

b6e6a22fce fix(#2402): honor response_language across orchestrator output + UAT checkpoint renderer (#2457)
* fix(#2402): honor response_language across orchestrator output + UAT checkpoint renderer

Replays the in-flight bot branch fix/2402-response-language-orchestrator-coverage
(seven commits, never pushed) onto current origin/next as a single squashed commit.
The original work was substantial and correct; this commit preserves its full scope,
trimmed where rebase conflicts + workflow size budgets required it.

Three independent layers where response_language was being dropped are closed:

Layer 1 — orchestrator-facing directives across workflows. Adds the strong
"All user-facing output in this workflow MUST be presented in {response_language};
technical terms, code, paths, and subagent prompts stay in English" directive
to ~40 workflows that previously either lacked it entirely (verify-work,
new-project, new-milestone, quick, manager, and ~35 more) or carried only
the weak subagent-prompt-only form (plan-phase, execute-phase). The directive
covers narration between tool calls and banner output, not just the
AskUserQuestion prompts.

Layer 2 — UAT checkpoint renderer (src/uat.cts). buildCheckpoint now accepts
an optional responseLanguage parameter and renders the frame strings
("CHECKPOINT: Verification Required", "Type `pass` or describe what's wrong.")
in any of 9 languages (English/Spanish/French/German/Portuguese/Japanese/
Chinese/Korean/Italian) with an alias table covering ~30 input variants
(en, es, español, ja, 日本語, etc.). cmdRenderCheckpoint reads
config.response_language via loadConfig(cwd) and passes it through, so the
byte-for-byte block verify-work.md reprints verbatim is already localized
when written — preserving the anti-injection hygiene rule at verify-work.md
(the model is forbidden to translate after the fact). CJK display width is
computed by East Asian Width property ranges (W/F) so the right ║ border of
the banner stays aligned for full-width characters. English fallback is
byte-identical to the pre-fix behavior when response_language is unset or
unrecognized.

Layer 3 — literal English report templates in execute-phase. The top-of-
workflow directive covers all template sites (templates are a structural
source, not literal output). Inline render-language notes that previously
sat at each template site were removed during the squash because they
pushed execute-phase.md over its frozen pre-phase-6 byte ceiling
(93600 — ADR-857 Phase 6 capstone). The single top directive covers the
same surface with fewer bytes.

Also extends src/docs.cts and src/init.cts to propagate response_language
into the init JSON bundle of the additional workflows so the directive can
read it.

Tests added:
- tests/uat.test.cjs: buildCheckpoint with unset/unrecognized language falls
back to English default; recognized language swaps only the two frame
strings while structural lines stay untouched; CJK display-width regression
(independent recomputation of East Asian Width W/F ranges).
- tests/workspace.test.cjs, tests/docs-update.test.cjs: response_language
  wiring through docs.cts/init.cts.

References: #2402; reporter's three-layer triage + Layer-4 follow-up; the
byte-for-byte anti-injection hygiene rule at verify-work.md (the reason
Layer 2 must be renderer-side, not model-translated).

This is a squash of the in-flight bot branch — seven commits representing
the original implementation plus its subsequent fix/CJK-padding/test/
changeset/regen cycles, none of which were ever pushed or PR'd. The squash
captures the final coherent state.

* chore(#2402): backfill pr:2457 in .changeset/2402-response-language-orchestrator-coverage.md

* chore(#2402): regen golden + size baseline after rebase against #2315 (PR #2451)

Rebase conflicts were entirely in generated artifacts (golden-install-parity
fixtures + workflow-size-baseline.json). After taking theirs during rebase,
regenerated cleanly against the merged source tree.

a62079b2da fix(#1865): runtime launcher honors CLAUDE_CONFIG_DIR (#2024)
* fix(#1865): runtime launcher honors CLAUDE_CONFIG_DIR

The gsd_run preamble resolved the Claude global install only at
$HOME/.claude/gsd-core/bin/, but the installer honors CLAUDE_CONFIG_DIR —
so a global install redirected via CLAUDE_CONFIG_DIR was invisible to every
gsd_run call (every command failed with 'gsd-tools.cjs not found').

The Claude resolver arm now uses ${CLAUDE_CONFIG_DIR:-$HOME/.claude},
matching the installer + the other runtimes' ${VAR:-default} pattern.
Default $HOME/.claude behavior is unchanged.

- _runtime-launcher.snippet.sh: Claude arm honors CLAUDE_CONFIG_DIR.
- sync-runtime-launcher.cjs re-run: 95 workflows/agents re-synced.
- review.md / discuss-phase.md: trimmed to stay under their byte budgets.
- runtime-launcher-parity.test.cjs: (A) substring updated for the new form
+ explicit #1865 assertion that the snippet honors CLAUDE_CONFIG_DIR.
- goldens + size baselines recaptured.

Closes #1865

92091d71f2 fix(#1871): wire phase archival end-to-end (phases archive cmd + default + atomic) (#1924)
Follow-up to #1919 (archive-then-remove core). Closes the remaining #1871
acceptance criteria so phase history is preserved across the full milestone
lifecycle, not just at phases.clear:

- #2 src/milestone.cts + src/phases-command-router.cts: extract shared
archivePhaseDirectories() helper; add cmdPhasesArchive (the previously
half-wired phases.archive alias now routes instead of erroring Unknown).
- #4 gsd-tools.cjs + src/milestone.cts: milestone complete archives phase
dirs by default (--no-archive-phases opts out; --archive-phases is now a
harmless no-op). complete-milestone.md updated to drop the redundant manual
  Yes/Skip archive prompt.
- #3 gsd-core/workflows/new-milestone.md: §6 stages the archive move + source
removal (git add .planning/milestones/ .planning/phases/) in the same commit
as the milestone start, so the archive lands atomically — no orphaned
uncommitted deletions, no un-archived dirs inherited.
- docs/CLI-TOOLS.md (+ ja/zh/ko/pt) + help/modes/full.md: flag accuracy.
- tests: phases archive command (#2) + milestone complete default archive /
  --no-archive-phases opt-out (#4). Goldens + workflow size baseline refreshed.

77c7b4fc9d fix(#1522): enforce canonical verification before phase transition (#1548)
* fix: require fresh phase verification before transition

* no-mistakes(review): Fix canonical verification closeout gates

* no-mistakes(review): Fix verify-work frontmatter promotion command

* no-mistakes(review): Fix stale verification gates

* no-mistakes(review): Fix canonical verification routing gates

* no-mistakes(review): Fix verification dependency and runtime routing gates

* no-mistakes(review): Block stale verification bypasses

* fix: handle large init manager outputs in verification workflows

* chore: update changeset pr number

* fix(verify-work): use fresh verification.status for stale gate

The stale check after UAT used phase_completion.verification_status from
session-start INIT while human_needed promotion already queried fresh
verification.status. Align the stale gate with the canonical query so
mid-session verification refresh is not ignored.

* fix(init): skip roadmap-checked phases when selecting next_phase

Roadmap-only phases without a disk directory were still promoted to
next_phase when their checkbox was already checked. Exclude
checkboxComplete phases so progress routing does not point at work the
roadmap already marks done.

* fix: gaps_found not overridden by stale, transition uses canonical verification

- verification.cts: check gaps_found before stale so gap-closure routing
is not masked by a newer summary mtime
- phase.cts: remove redundant findStaleVerificationSummary — readVerificationStatus
  already handles stale detection
- transition.md: replace raw grep on file content with verification.status query
to avoid false-positive blocks from body text matching

* ci: retrigger tests after rebase

* fix(transition): replace gsd_run advisory check with awk frontmatter extraction

The runtime launcher is not defined until the update_roadmap_and_state step
bash block (~line 165). The early verify_completion block used gsd_run to
query verification.status, which violated the runtime-launcher-parity test:
'preamble appears AFTER the first gsd_run reference'.

Replace the gsd_run call with an awk-based frontmatter extractor that reads
only the status: field between the two --- fences. This avoids both the
preamble-ordering constraint and the original false-positive grep bug where
body text like 'previous_status: gaps_found' would match a full-text regex.

The phase.complete gate at update_roadmap_and_state is the canonical
enforcement point; this early check is advisory only.

Also update workflow-size-baseline.json for the updated transition.md size.

Fixes: runtime-launcher-parity test (B)

* fix: re-check verification under planning lock in phase complete

Move readVerificationStatus into withPlanningLock so stale verification
cannot slip through when a SUMMARY.md is written between the gate and
the roadmap/state mutation. Return the blocked status from the lock
callback and emit the error after release to avoid leaving .lock behind.

* fix(transition): gate on canonical verification.status including stale

Replace awk frontmatter read with verification.status query so transition
blocks when summaries are newer than VERIFICATION.md, matching phase.complete
and other workflows (autonomous, progress, verify-work).

* Fix workflow verification gates for yolo transition and stale routing

Require VERIFY_STATUS passed before yolo/interactive transition advance.
Route stale verification recovery to verify-work, matching canonical projection.

* fix(transition): use verification.status query for stale-aware advisory check

The awk-based check read raw frontmatter status: passed, which misses the
stale case where summaries are newer than the VERIFICATION.md file even
though the frontmatter still says passed. The stale status is computed from
file modification times, not stored in frontmatter.

Move the preamble to the verify_completion bash block (the first block with
a gsd_run call) so gsd_run query verification.status can be used for the
advisory check. This gives the full readVerificationStatus logic including
mtime-based staleness detection, matching the enforcement gate at phase.complete.

Capture full JSON (VERIFY_JSON) so next_action can be included in the
advisory output alongside the status.

Also update workflow-size-baseline.json for the updated transition.md size.

* ci: trigger test matrix for 525b946

Co-authored-by: Codesmith <codesmith-bot@users.noreply.github.com>

* fix(transition): restore awk frontmatter extraction for pre-shim verification check

The gsd_run launcher shim is not defined until line ~163 of transition.md,
so the verification debt check at line ~80 cannot use gsd_run. Restore the
awk-based frontmatter extraction that correctly reads status without needing
the runtime, and restore the shim at its proper location before
phase.complete.

* fix(#1522): clarify transition verification gate wording

* fix(#1522): update transition workflow size baseline

* fix(#1522): update workflow-size-baseline after rebase onto next

Co-authored-by: Codesmith <codesmith-bot@users.noreply.github.com>

* fix(#1522): guard findStaleVerificationSummary FS calls + thread opts.fs seam (review)

Address review blocker B1 on #1548: findStaleVerificationSummary ran fs.readdirSync
and two fs.statSync calls unguarded between readVerificationStatus's try/catch sections,
so a TOCTOU race (a SUMMARY listed by scanPhasePlans then removed before statSync) or any
FS error threw uncaught into callers NOT under the planning lock (init.manager /
init.progress / uat-predicate). Wrap the body in try/catch degrading to 'not stale', and
thread the injectable opts.fs seam (add statSync to FsLike, pass fsImpl from the caller)
for parity with readVerificationStatus's no-throw contract and testability. Also adds the
Verification Module glossary entry to CONTEXT.md (review B3).

e9f9ae49c8 fix(#1146): single base-branch resolver across forking workflows (#1198)
* fix(#1146): single base-branch resolver across forking workflows

Replaces duplicated per-workflow bash detection that silently fell through
to :-main on repos where origin/HEAD is unset (git init+remote add+fetch
without set-head, most CI checkouts, many worktrees).

New CJS module git-base-branch.cjs exposes `gsd_run query git.base-branch`
with full precedence ladder: git.base_branch config override → origin/HEAD
symref → git remote show origin (authoritative) → local branch presence →
"main". All git subprocesses bounded with timeouts; degrades gracefully.

Wires execute-phase, quick, ship, complete-milestone, and pr-branch to the
single resolver. Removes 14 lines of duplicated detection bash across the
five workflows.

Includes 7 behavioral tests covering the full precedence ladder including the
key regression case (master repo, origin/HEAD unset → must return "master",
NOT "main") and an anti-regression guard that fails if any workflow
re-introduces the :-main/:-master fallback pattern.

* test(#1146): add tests for flat base_branch config key and both-branch tie-break

Closes two mutation gaps identified in adversarial review:
- A2: flat {base_branch: ...} at config root (legacy key form) was covered
by code but unguarded against mutation of lines 74-75 in resolver
- H: tier-4 tie-break when both main+master exist locally (main wins,
per tryLocalBranch JSDoc) was documented but untested

9/9 tests pass.

* fix(#1146): degrade gracefully when gsd_run unavailable in handle_branching bash blocks

handle_branching (execute-phase.md) and step 2.5 (quick.md) are extracted
and run verbatim by behavioral tests that lack the gsd_run preamble.
Adding a || fallback ladder (git symbolic-ref then echo main) keeps the
unified resolver as primary in real workflows while letting the test harness
succeed without gsd_run defined.

Also propagates updated runtime-launcher preamble to pr-branch.md (added in
origin/next MemPalace PR) and regenerates workflow-size-baseline.json.

7c07fce70f fix(#381): make gsd_run launcher reachable in fresh-shell-per-block runtimes (#1084)
* fix(#381): make gsd_run launcher reachable in fresh-shell-per-block runtimes

On runtimes that execute each fenced bash block in a separate shell process
(e.g. Claude Code — documented behavior: each Bash command is a separate
process; inline shell functions and exported vars do not persist between
calls), the once-per-file gsd_run() function was undefined in every block
after the preamble block, and the call was swallowed by
`2>/dev/null || echo "{}"` into silent empty state.

Fix (budget-neutral session-level resolution):
- Ship gsd-core/bin/gsd_run, a POSIX sh wrapper that symlink-resolves its own
location and execs the co-located gsd-tools.cjs. Exposed on PATH via the npm
`bin` field (global installs) and shipped to local installs via the recursive
  gsd-core/ copy.
- The per-file launcher preamble now appends `export PATH='<bindir>':"$PATH"`
to the file named by $CLAUDE_ENV_FILE (Claude Code's documented
env-persistence mechanism) so later fresh-shell blocks resolve gsd_run from
PATH. Guarded as a strict no-op when CLAUDE_ENV_FILE is unset; the inline
gsd_run() definition remains the fallback for all other runtimes. The
single-quoted dir neutralizes shell metacharacters at source time.
- Propagated via scripts/sync-runtime-launcher.cjs to all launcher-using files.
- XL workflow byte budget 93000 -> 93200 (the ~130B clause pushes plan-phase.md
to 93135; legitimate content growth, ratchet-up per #717).

Regression tests (I)/(J) in runtime-launcher-parity.test.cjs cover wrapper
delegation and end-to-end PATH persistence (sourcing the env file with a
space-bearing install path).

Known limitation: an install path containing a literal single-quote yields a
malformed env-file line and falls back to the status quo (no regression);
rare on sanitized home directories.

Closes #381

* docs(#381): add changeset for gsd_run fresh-shell reachability fix

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* test(#381): scope test (J) bare-PATH execution to POSIX (Windows Git Bash exec bit)

Windows Git Bash (msys2) does not honor Node's chmod exec bit for
PATH-executing extension-less scripts, so the bare `gsd_run` command lookup
failed there even though the env-file PATH persistence was correct. The
env-file content assertions (the fix's actual cross-platform logic) still run
on every platform; only the final source-and-execute sub-step is gated to
non-win32. Global installs on Windows are covered by npm's generated bin shim.

5e8a723089 feat(templates): add optional Business Context section to PROJECT.md template (#756)
* feat(templates): add optional Business Context section to PROJECT.md template

Adds an optional `## Business Context` section (Customer, Revenue model,
Success metric, Strategy notes) between Core Value and Requirements, for
monetized or customer-facing projects. Optional by default — an HTML comment
tells non-business projects to delete it; capped at four one-line fields to
stay a constraint reference, not a business plan. The milestone evolution
review in complete-milestone.md checks it only when the section is present.

Refs #72

* chore(changeset): set pr number for #72 fragment

* test(#72): add source-text-is-the-product exemption marker

Addresses review Minor #1 on PR #756. The contract test reads the
PROJECT.md template and complete-milestone workflow .md files and
asserts on their content (the local/no-source-grep pattern). Those
.md files ARE the product surface, so this is a valid
source-text-is-the-product case. Add the explicit // allow-test-rule
marker per RULESET.TESTS.no-source-grep.exemption so intent is
audit-traceable before the rule promotes to error (#453).

a90c654745 fix(#891): probe non-Claude runtime homes in gsd-tools launcher shim detection (#911)
- Updated `gsd-core/workflows/_runtime-launcher.snippet.sh` with 15 new
`elif` arms covering Hermes, Cursor, Codex, Gemini, Copilot, Windsurf,
Augment, Trae, Qwen, CodeBuddy, Cline, Grok, Antigravity, OpenCode, and
  Kilo (respecting each runtime's env-var override with a `$HOME`-relative
  default).
- Re-ran `scripts/sync-runtime-launcher.cjs` to propagate the expanded
snippet into all `gsd-core/workflows/*.md` files (~70 files).
- Manually applied the same snippet update to `commands/gsd/import.md`
  (1 occurrence) and `commands/gsd/graphify.md` (5 occurrences) — these
are not covered by the sync script.
- Updated `tests/workflow-size-budget.test.cjs` budgets (XL/LARGE/DEFAULT
+ discuss-phase target) to account for the ~3 KB snippet expansion.
- Added regression test `tests/bug-891-non-claude-runtime-home-fallback.test.cjs`
(6 tests: structural probe presence, ordering, behavioral HERMES_HOME
env-var + default-path stubs, resolution order, and workflow propagation).
- Added `.changeset/891-launcher-non-claude-runtime-homes.md` (Fixed).

463cffd894 chore(#604): rename get-shit-done/ runtime directory to gsd-core/ (#615)
* chore(#604): rename get-shit-done/ runtime directory to gsd-core/

Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the
on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary
(`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers
are unaffected.

Mechanical (bulk, ~90% of the diff):
- `git mv get-shit-done gsd-core`
- Swept path/identifier references across the repo via
`perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead
preserves the five legitimate slug variants that are NOT the directory:
  get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names).
- Build/manifest wiring: package.json (bin, files, coverage globs),
  tsconfig.build.json (outDir), ~86 .gitignore build-output entries,
  stryker.config.mjs, scan-ignore files, install.js path strings.
- Frozen (not rewritten): CHANGELOG.md history; translated docs
  (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/).

New logic (review here):
- src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper
ADR-0008 installer migration. On upgrade it walks the legacy
`~/.claude/get-shit-done/` tree, classifies each file via the prior install
manifest, and emits remove-managed / backup-and-remove for managed files
while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked
root and symlinked entries; bounds-checks every path under configDir). The
framework rolls back on install failure. Emptied dirs may remain (framework
has no recursive dir-removal primitive) — documented.
- scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare
`get-shit-done` directory token (split token to avoid self-match; case-
insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists
CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines).
Wired into the lint-tests CI job.
- Restored scripts/lint-package-identity-drift.cjs detection regexes (the
mechanical sweep had wrongly rewritten the old-name patterns it exists to
detect) and marked them as intentional legacy references.
- TDD tests for the migration and the guard; do.md slash-command guard regex
  tightened so a `/gsd-core/bin` path segment is not mistaken for a command;
  changeset + docs/installer-migrations.md row added.

Breaking: the installed runtime path moves `~/.claude/get-shit-done/` ->
`~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed
files (preserving user files) on upgrade. Users with custom hooks/configs
hardcoding the old path must update them.

Closes #604

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): unsweep pending changesets + allowlist injection-example docs

CI fixes for the rename PR:
- Do not sweep pending .changeset/*.md (ephemeral release-note fragments,
like CHANGELOG); reverted those body edits so 5 pre-existing malformed
fragments (missing type/pr) no longer enter the PR diff and trip docs-lint.
Allowlisted .changeset/ in the legacy-name guard accordingly.
- Allowlisted TEST-EXAMPLES.md and docs/explanation/security-model.md in
prompt-injection-scan.sh: they contain intentional injection examples /
security-model prose; the path-reference rewrites are kept.

CodeQL alerts on this PR are pre-existing (alert lines unchanged by this PR;
none in the new migration/guard) and are out of scope for the rename.

* fix(#604): resolve CodeQL alerts surfaced on this PR

The rename diff touched files carrying pre-existing CodeQL findings; per the
no-pre-existing-dismissal rule, fixing every surfaced alert rather than waving
them off. All behavior-preserving:

- scripts/ci-test-scope.cjs: build the config-path match from string
.includes() instead of a RegExp over an arg-derived value (js/regex-injection).
- src/profile-output.cts: escape backslashes before pipe-escaping desc/safeName
so the table-cell escape is complete (js/incomplete-sanitization).
- tests/{bug-2643,bug-2808,docs-parity-live-registry}: two-pass HTML-comment
  strip so a bare/unclosed `<!--` cannot survive (js/incomplete-multi-character-sanitization).
- tests/inline-plan-threshold: drop the no-op `\s`->`\s` identity replace,
keep the meaningful POSIX-class conversion (js/identity-replacement).

Verified: build:lib green; the touched test files + ci-test-scope + profile-output
suites pass; lint:legacy-name clean.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): correctly resolve remaining CodeQL alerts (regex-injection + sanitization)

The prior commit's fixes for two alerts were ineffective:
- ci-test-scope.cjs js/regex-injection: the alert is the CLI-arg-derived `file`
reaching static regex `.test(file)` calls (not the config rule). Removed ALL
regex over file/t — startsWith/includes/=== string checks + an isWindowsHint
helper — so there is no regex sink for the tainted value.
- js/incomplete-multi-character-sanitization (3 test files): a single
  `.replace(/<!--...-->/g,'')` can let `<!--` re-form. Replaced with a fixpoint
loop (replace until stable) plus a final bare-opener strip.

Verified: no regex over file/t remains; ci-test-scope + the 3 test suites pass;
lint:legacy-name clean.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): make ci-test-scope + comment-strippers regex-free to clear CodeQL

CodeQL flags the regex PATTERNS syntactically (regex-injection on the
--files arg split; incomplete-multi-character-sanitization on the <!--...-->
replace), so loop fixes do not satisfy it. Made these paths regex-free:
- ci-test-scope.cjs splitFiles: char-by-char separator tokenizer (no /[,\\s]+/).
- 3 test files: indexOf/slice HTML-comment stripper (no .replace(/<!--/)).
Behavior preserved; ci-test-scope + the 3 suites pass; guard clean.

* fix(#604): unblock security base64 scan on the large rename diff

The security job hit its 10m timeout: base64-scan.sh choked on the binary
test fixture tests/feat-3594-parser-property-style.test.cjs (embedded NUL/
non-UTF8 bytes -> thousands of bogus blobs + "ignored null byte" warnings),
and the ~800-file rename diff is slow to scan regardless.

- scripts/base64-scan.sh: skip binary-by-content files (grep -Iq .) — they
can't carry base64-obfuscated *text* and feeding NUL bytes through the
per-line scanner is pathologically slow. collect_files already filtered
binary *extensions*; this catches binary *content* in text extensions.
- .github/workflows/security-scan.yml: raise the security job timeout 10m->30m
to accommodate very large diffs (the scan itself is unchanged).

Verified locally: scan skips the fixture, 0 "ignored null byte" warnings,
0 findings, exit 0.

* fix(#604): sweep get-shit-done refs introduced by merging next

The branch was updated with next (#614/#384/#618 etc.), which reference the
get-shit-done/ dir (still named that on next). Swept the stale references in
the merged files to gsd-core so the rename stays consistent and lint:legacy-name
passes:
- commands/gsd/discuss-phase.md (runtime-launcher shim paths)
- src/core.cts (getAgentsDir layout comments)
- tests/bug-384-agents-runtime-aware.test.cjs (require path to runtime lib)

Verified: guard 0 violations; build green.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#604): exclude gsd-core/ path segments from bug-3683 command cross-ref invariant

The #614 runtime-launcher shim added to discuss-phase.md references
`${_GSD_RUNTIME_ROOT}/gsd-core/bin/...`. bug-3683's REF_PATTERN excluded path-y
refs only via lookbehind, but `}` precedes `/gsd-core/` in the shim, so it
mis-read the directory path as a dangling `/gsd-core` command ref (same class as
the #604 bug-2954 fix). Added a trailing `(?![\w-]*\/)` so `/gsd-<x>/...` path
segments are not treated as slash-command references.

Verified locally on BOTH platforms before pushing:
- mac (node 26) full suite: 0 failures
- gsd-test-runner (linux, node22 image) full suite: 0 failures
- bug-3683 + bug-2954 pass.

* fix(#604): lazily resolve findProjectRoot in gsd-tools (harden flaky CI)

CI intermittently failed state.test's gsd-tools subprocess with
"findProjectRoot is not a function" (flip-flopping across legs; not reproducible
on mac full suite, gsd-test linux full suite, test:unit, or state.test x8).
findProjectRoot is a re-export from core.cjs (sourced from project-root.cjs);
binding it via destructure at module-load can be undefined under a load-ordering
edge. Resolve it lazily at call time via a small wrapper so the lookup happens
after core.cjs is fully initialized.

Verified green on BOTH platforms before pushing:
- mac (node 26) full suite: 0 failures
- gsd-test-runner (linux, node22) full suite: 0 failures
- state.test.cjs: 106/106; gsd-tools loads cleanly.

* fix(#604): allowlist verification-patterns.md placeholder examples in secret scan

The rename git-mv'd references/verification-patterns.md into gsd-core/, pulling
it into the secret-scan diff. It documents stub/placeholder RED-FLAG env-var
examples (illustrative Stripe test-key / database-URL / API-key placeholders) —
not real credentials. Added it to .secretscanignore with the strict annotation,
mirroring the existing gsd-core/workflows/plan-phase.md exception.

Verified locally: secret-scan-lint --strict OK; secret-scan --diff origin/next
exits 0 with 0 findings.
