bd31cf2bba fix(#3321): exclude .claude/.planning from no-phantom-issue-refs SKIP_DIRS (#3396)
* test(#3321): add failing-first regression proving walk() must skip .claude/.planning

RED step: SKIP_DIRS does not yet exclude .claude/.planning, so this test
is expected to fail until the next commit adds them.

* fix(#3321): exclude .claude/.planning from no-phantom-issue-refs SKIP_DIRS

walk() previously had no guard against descending into ambient,
gitignored .claude/worktrees/** or .planning/** content, so this
guard's pass/fail would have depended on the developer's local
worktree layout the next time PHANTOM is repopulated. Currently
dormant (PHANTOM=[] short-circuits via t.skip before walk() runs)
but the gap was real, per #1885 F23.

* docs(#3182): ADR-3180 — planning semantic model single owner

Phase 0 design lock for epic #3180. Names one canonical owner per
semantic derivation, specifies the frozen-enum scope contract that
distinguishes a genuinely-empty computation from a truncated or
unscoped one, and locks the drift-guard contract.

Ships no production code. Phases 1-5 execute against this ADR.

* test(#3182): prune real issue 3182 from phantom-ref guard, fix empty-list regex

The guard's own header documents that its list rots: entries are phantom
only until the repo's shared issue/PR counter reaches them, and once the
counter passes an entry it must be deleted. Creating the Phase-0 sub-issue
advanced the counter past 3182, so the guard began rejecting a legitimate
citation of a real issue - the failure its header already records happening
twice, with PRs 2551 and 2361.

3182 was the last entry, and removing it exposed a latent bug: the regex
builder interpolated the list unconditionally, so an empty list yields
(?:#(?:)\b)|(?:issues/(?:)\b), whose empty alternation matches every issue
reference in the repo. Following the file's own maintenance instruction
would have turned a green guard into one failing on nearly every file.

buildRefRe() now returns null for an empty list and is exported, with
boundary coverage at 0/1/2 entries plus word-boundary and bare-digit
negative cases.

7dd9e59f6b test(#3090): stop exempting violations under categories that do not fit
An allow-test-rule annotation citing a category that does not apply is worse
than no annotation, because it reads as reviewed. Eight were confirmed by
reading the assertions each one covered, and auditing the rest found five more
plus one refutation — a converter test whose wording described the wrong
mechanism while the covered assertion genuinely was deployed-text.

The instructive one used the CANONICAL string for the same mistake: STATE.md
command output labelled as a deployed artifact. A canonical string is not
evidence the category fits, which is why normalising strings alone would have
laundered the problem rather than fixed it. Every mapping the audit had inferred
rather than code-verified was spot-checked before rewriting, and the ones that
turned out not to fit were re-annotated rather than relabelled.

Fourteen STATE.md assertions had a typed extractor available all along and now
use it; their annotations came out because nothing needs exempting. Eight
assertions genuinely need a production change first — CLI stdout and stderr with
no structured mode — and are tagged pending-migration-to-typed-ir citing #3090,
which is what that category is for. It had zero real uses before this, while one
file carried a real citation to migration issue #2974 under a non-canonical tag.

Six annotations covered assertions that do no text matching at all. An exemption
for a violation that does not exist is noise that makes the real ones harder to
audit; those are removed.

atomic-write-coverage gains the annotation it always warranted — its own
docstring describes a structural-regression-guard while the file carried none.

Fifty-nine non-canonical strings across roughly thirty files are normalised, and
the allow-test-rule allowlist is regenerated to match. 472 annotations became
463: every one now uses a canonical category, and the two remaining
non-canonical strings are ESLint RuleTester fixtures, not annotations.

* fix(#2653): regenerate stale api-coverage.cjs + add artifact-sync guard

The tracked build artifact gsd-core/bin/lib/api-coverage.cjs had drifted
four days behind src/api-coverage.cts: PR 2551 landed the #2366 fix in
the source without regenerating the compiled output, so the module that
actually ships still carried none of it.

Regenerate the artifact, and add scripts/lint-compiled-artifact-sync.cjs
to lint:generated-sync so a tracked compiled artifact can never again
silently diverge from its source. The check derives its file set from
git ls-files rather than a hand-maintained list, and is regime-agnostic:
if these artifacts are later untracked and gitignored per ADR-457, the
tracked set becomes empty and the check passes trivially.

Verified fail-first: the guard exits 1 against the previously-committed
artifact (37731 bytes vs 38634 expected) and 0 after regeneration.

Also fixes a defect this change surfaced in tests/no-phantom-issue-refs:
its PHANTOM list still banned 2551 and 2361, but GitHub numbers issues
and PRs from one shared counter and this repo has since reached 2654, so
both now resolve to merged PRs. The guard was rejecting accurate
citations of them — it failed this very commit for naming PR 2551 as the
drift's provenance. Verified by replaying the guard's scan: 1 offender
under the old list, 0 under the pruned one. Only 3182 is still a 404.

48687521db fix(#1545): make no-phantom-issue-refs walk() robust to broken symlinks (#1546)
walk() collected dirents by extension only, then readFileSync'd each — so a
broken symlink named *.md (e.g. the gitignored CLAUDE.md worktree symlink whose
target is absent in a gsd-test Docker copy) crashed the #1073 guard with ENOENT.
It passed on a developer Mac (the symlink resolves) and in GitHub CI (CLAUDE.md
is gitignored/absent), failing only on the gsd-test-docker-from-worktree path —
a real error hiding behind the local environment, not a flake.

Guard the collection with entry.isFile() so symlinks (and other non-regular
dirents) are skipped deterministically on every platform; gitignored files are
not shipped repo text, so excluding CLAUDE.md is correct. Add a deterministic
regression test (dangling *.md symlink fixture, Windows-symlink-guarded with a
genuine t.skip) asserting walk() excludes the broken symlink and the read loop
never throws ENOENT.

Closes #1545.

3a3b2135c2 chore(#1073): purge phantom pre-migration issue refs from source, tests, docs (#1471)
#2551/#3182/#2361 are pre-migration get-shit-done-redux issue numbers with no
equivalent in open-gsd/gsd-core; they mislead triage and manufacture phantom
blockers. Repoint to real successors (#717 byte-budget rework, #720) or rewrite
as prose referencing the discuss-phase/modes progressive-disclosure split.
Correct co-located 'line budget'/'<500 lines' framing to the byte-based reality
(#717). Add a CI guard (tests/no-phantom-issue-refs.test.cjs) that fails if a
phantom ref is reintroduced. SSH-key patterns (id_ed25519) left untouched.
No user-facing runtime behavior change.
