fix(#4378): mint collision-free SEED-YYMMDD-xxx seed ids instead of a shared count (#4754)
* test(#4378): regression tests for collision-free seed ids * fix(#4378): mint collision-free SEED-YYMMDD-xxx ids, not a shared count plant-seed derived the next seed id from 'ls .planning/seeds/SEED-*.md | wc -l'. .planning/seeds/ is shared but each worktree only sees what has merged, so two workstreams planting before either merges computed the same id and git merged both files silently. The id is now the local date plus a 3-char random base36 suffix -- the shape .planning/quick/ already uses -- computed from knowledge one worktree has alone, with a same-day regen guard. deriveSeedIdentity learns the new canonical grammar alongside legacy SEED-NNN (whose parsing never changes), the --enrich parser and the filename-prefix fallback keep the full new-format id, and the docs that state the filename shape move to it. The prefix fallback previously truncated any non-pure-numeric id at 'SEED-<digits>' -- the same one-id-two-answers ambiguity the issue reports, reproduced one level down. * fix(#4378): harden seed id generation per adversarial review - parse-idea: anchor the --enrich extractor to the flag and capture the complete id, uppercase-tolerant; a leftmost 'SEED-[0-9]+' truncated an uppercase or malformed suffix to its date and enriched an arbitrary same-day seed via head -1. Pin the verified values. * docs(#4378): backfill changeset PR number * fix(#4378): audit seed identity uses the canonical grammar Review of this PR found the audit surface publishing a fused filename stem (SEED-081-region for SEED-081-region.md) where list-seeds reports the canonical id -- one id, two answers across surfaces, the same ambiguity class the issue files. scanSeeds now derives identity through the SAME deriveSeedIdentity the list-seeds gate uses (frontmatter id, then filename id-prefix, then stem), and audit-open acknowledge resolves --seed-id by scanning for the derived identity, falling back to the literal stem so callers scripted against pre-canonical output keep working. Roll-in per the fix-inline rule: found during this PR's review, same seed-identity seam. RED probe: pre-fix audit published seed_id SEED-081-region-becomes / slug 081-region-becomes for a legacy seeded file; post-fix SEED-081 / region-becomes, matching list-seeds. * test(#4378): probe timeout uses the class norm after windows-lane timeout The windows conformance shard failed its bounded sh -c probes at the local 5000ms bound (cold sh.exe spawn under shard load) while the identical code passed this PR's two earlier windows waves. the same sunset that removed the gemini RUNTIME in #1928 (shipped 1.8.0). GSD
targets solo developers, so those tiers ARE the user path: the lane spawned
`gemini {{model}} -p -`, a binary that no longer answers for the majority of
users, and five locales documented it as a supported choice.
The lane was re-created after #1928 by the reviewer-lane-as-manifest-data work

* test(#4324): prove colon tokens the installer cannot convert leak Failing-first regression coverage for #4324. The install rewrite (transformContentToHyphen) is gated on an exact match against the commands/gsd stem list, so any /gsd:<token> whose token is not a registered stem survives the install and reaches the user as the deprecated colon form. The gate is load-bearing -- it is the only thing protecting the workflow DSL marker family (gsd:section, gsd:protected, gsd:loop-host, gsd:guard, gsd:dispatch, gsd:plan-revision-conflicts), which workflow-fragments parses as a literal. So this suite asserts the shipped text is convertible rather than asserting the transform is broad, and pins the marker family as explicit negative space. * enhance(#4139): Phase 8 — the toggle becomes discoverable, and the ledger closes ADR-4139's final phase. workflow.compact_content already defaulted to false (Phase 1's buildNewProjectConfig hardcoded default), but nothing surfaced it: /gsd-new-project never asked, and /gsd-settings/config had no toggle path for an already-initialized project — config-set/config-get were the only route. new-project.md gains a fourth question in the existing Round 2 AskUserQuestion array (grouped with the other general-workflow-behavior toggles, not the per-agent capability questions above it) and threads compact_content into the config-new-project CLI JSON literal. settings.md mirrors the exact pattern every other non-capability workflow.* key already follows: read_current bullet, question block, update_config write, the safe-merge non-capability-keys list, save_as_defaults, and the confirm summary table — seven edits, zero new src/*.cts code, since Phase 1's merge logic is a generic passthrough. Its success_criteria question-count ("24 settings") is bumped to 25 to match the now-25-entry main AskUserQuestion batch. settings-advanced.md deliberately does NOT get a duplicate question: no other boolean toggle in this repo is asked in both settings.md and settings-advanced.md, and there's no reason to start with this one. docs/CONFIGURATION.md, docs/USER-GUIDE.md, and a new docs/features/4139-compact- content.md fragment (regenerated into docs/FEATURES.md) document the toggle. ADR-4139 itself: Status flips Proposed -> Accepted, the acceptance-criteria section becomes a guard ledger — a 13-row table covering all 12 of #4139's original checkboxes plus the shipped-content guard criterion, each with real evidence (the merged PR that satisfied it, fetched via `gh issue view --json closedByPullRequestsReferences` rather than asserted from phase numbers) — and both "Open questions for the implementation phases" are resolved rather than left dangling: discuss-phase was never converted to spine+detail shape (verified: no detail/ subdir exists) — a genuine gap, not a reasoned decline; the disjointness check is confirmed line-based by reading compact-content-split.cjs's normalizeNonTrivialLines directly. Orthogonal review (isolated Standards/Spec code-review + security-review sub-agents) found and this fixes two real defects: the changeset fragment's body didn't match CONTRIBUTING.md's single em-dash-sentence format (was multi-sentence prose naming implementation file paths); and settings.md's own success_criteria still said "24 settings" after the new question pushed the main batch to 25. Also fixed, found by the Spec pass while confirming commands/gsd/settings.md correctly needed no sync edit: that file and its skills/gsd-settings/SKILL.md twin both still described "Interactive 5-question prompt (model, research, plan_check, verifier, branching)", stale since long before this phase (the batch has had far more than 5 questions for a while) — replaced with a description that names the current set without hardcoding a count that will drift again. gsd-test (real run, sha 1da78fe2) caught a third real regression the local sweep missed: new-project.md is a registered spine+detail split for Phase 4's token-reduction benchmark (scripts/benchmark-compact-content.cjs), and the new question's +167 tokens drifted the committed baseline (tests/fixtures/compact-content-benchmark-baseline.json). The benchmark itself is designed never to fail CI on drift, but the test asserting the COMMITTED baseline is currently non-drifted correctly caught it. Regenerated via `node scripts/benchmark-compact-content.cjs --write`; re-verified --check now reports "up to date" and the test file passes 27/27. Closes #4408. Closes #4139. Emitted-Drift-Ack-Growth: new-project.md — new 4th Round-2 AskUserQuestion entry (Compact Content, #4139) plus the config-new-project CLI JSON field and explanatory sentence; a new opt-in toggle needs new prose. * test(01-01): define reviewer-support trait contract Add failing coverage for step.supportsReviewerLanes (#4209 DISP-02): validator rejects non-boolean values with an exact field path, accepts missing/true/false, and the real code-review capability.json steps must declare supportsReviewerLanes: true. Add loop-resolver projection coverage proving the trait reaches activeHooks verbatim for a provider-neutral synthetic step (not code-review-specific), and that omitted/false values stay inert (no key on the active hook). All 8 new assertions fail today: the validator has no such field, and loop-resolver has nothing to project. RED before GREEN. * feat(01-01): declare reviewer-capable steps Add step.supportsReviewerLanes (#4209 DISP-02): a strict optional boolean opt-in trait, step-scoped (not capability-wide). Only a literal true validates and projects; false/omitted stay inert (no key on the projected active hook), and every non-boolean type fails capability-validator.cjs with an exact field-path error. Opt both existing code-review steps (execute:post, execute:wave:post) into the trait in capabilities/code-review/capability.json. That is exactly the "each workflow.md hand-wiring the call" the maintainer's redirect said to eliminate. File lines are now bare paths. * fix(#4209): resolve reviewer-lane trait in-process, fix CI failures found in review round 3 Opus critical-code-reviewer found a real Blocking defect in the --cap-id/ --point self-invocation added last commit: `dispatch-step` spawned `loop render-hooks <point> --raw` as a subprocess and bare-JSON.parse'd its stdout, but `io.cjs`'s output() redirects any payload over 50000 chars to `@file:<path>` instead of inline JSON -- the same overflow protocol this feature already unwraps for its OWN dispatch result 60 lines later in code-review.md. A large-enough activeHooks envelope (more installed capabilities/fragments) would throw, get silently swallowed by the bare catch, and misreport a real trait as trait_not_enabled with zero diagnostic. Fixed by extracting the config/registry/capability-state resolution `cmdLoopRenderHooks` already performs into an exported pure function, resolveActiveHooksForPoint (both `cmdLoopRenderHooks` and dispatch-step now share it), and calling it in-process from dispatch-step instead of spawning a subprocess at all. This eliminates the @file: exposure entirely (the dispatch-step path never touches the rendered-string envelope or its JSON-stringify/50000-char threshold), removes one subprocess spawn per code-review invocation, and gives a genuine diagnostic (stderr warning) on resolution failure instead of silent fail-closed. Corrected three doc/ docstring references to the now-removed subprocess self-invocation. Also fixes 2 real CI failures this round surfaced: - lint-tests: the agy-F1 control-char regex fix's `eslint-disable-next-line no-control-regex` comment was unused under this project's ESLint config (verified locally: the rule never actually flags \x00-\x1f in this repo's config) -- a mistake from an earlier commit this session, never actually lint-checked before push. Reads "relative to the base SHA above" now. * fix(#4209): make getLane/configGet/plan required deps, delete dead defaults R3/R4 from the review round I'd deferred as low-priority test-churn: this file's one production caller (gsd-tools.cjs's dispatch-step handler) always supplies all three, so the fallbacks were dead in production -- but each was actively WRONG if ever reached: the default configGet always returned undefined, silently disabling resolveLaneBudget's overflow guard; the default getLane looked up only first-party REVIEWER_LANES, diverging from production's overlay-merged roster; the default plan skipped per-host effort resolution entirely. These defaults were introduced by this PR's own earlier work (this file did not exist before #4209 -- first commit a760bfcda, 01-02), not inherited from elsewhere, so there's no external caller depending on the lenient contract. The external reviewer lane has no access to that persona at all -- it only ever sees buildSourceReviewPrompt's bounded text, which sent the bare depth label with zero definition to a third-party CLI with no other source of truth for what "standard" means. Added depthMeaning(), condensed from the internal reviewer's own <depth_levels> definitions so the two stay consistent, and interpolated it into the review-request sentence. 150/150 tests pass, tsc/lint clean. * fix(#4209): merge dispatch_reviewer_lanes' split fences into one shell invocation CR-01 (Opus critical-code-reviewer, confirmed by direct execution): the roster-matching fence set EXPLICIT_JOINED/EXPLICIT_REVIEWER_SLUGS, and a SEPARATE later fence read them via ${#EXPLICIT_REVIEWER_SLUGS[@]} to decide whether to dispatch at all. This file's own documented rule (its depth-resolution guard, stated explicitly a few hundred lines earlier) is that a guard and the extraction it protects must run as one shell control-flow decision, because markdown-fenced blocks do not share shell state -- this step violated its own file's rule for the entire feature's gating condition. Merged the roster-resolution fence and the dispatch-decision fence into one continuous bash block, removing the intervening prose that split them. Fixed the stderr-based failure detection in the same edit (RQ-01: checking whether stderr is non-empty misfires on any benign Node warning; now checks the actual exit status of the roster-resolution command). Verified by extracting the merged fence and executing it standalone, driving both branches: --codex resolves EXPLICIT_JOINED=codex, SLUGS_COUNT=1, and a real dispatch-step call succeeds; no flags resolves EXPLICIT_JOINED empty, SLUGS_COUNT=0, dispatch-step never invoked (COMP-01). 141/141 workflow tests pass, tsc/lint clean. * fix(#4209): depthMeaning accuracy, injection defense on all embedded fields, hoisted prompt write Batch of Required/Suggestion fixes from the Opus critical-code-reviewer + writing-for-agents pass: - CR-02/CR-03: depthMeaning() dropped real categories from quick (empty catch blocks, commented-out code) and deep (error propagation, state mutation consistency, circular dependencies) relative to the real <depth_levels> block, and had zero test coverage. Restored full accuracy and added tests that read the real agents/gsd-code-reviewer.md file directly, so drift between the two can't recur silently. Added an fs.realpathSync check (ENOENT-tolerant — a git-diff path can legitimately name a file already deleted in a stale worktree), realpathing repoRoot itself too so a symlinked repoRoot (e.g. /tmp on macOS) doesn't false-positive-reject its own real children. - WR-08: a comment in the per-lane loop still said a throwing writePromptFile() was caught there — stale since the prompt write was hoisted above the loop in an earlier round. WR-03 (validate depth against the quick/standard/deep enum) was considered and declined: this dispatcher is deliberately capability-neutral (see the existing "synthetic step context" test, which passes a non-code-review depth label on purpose to prove no code-review-specific special-casing exists). WR-01 (double registry load), WR-02 (trim-vs-hard-fail budget semantics), and WR-07 (reason omitted on the aggregate return) were verified against source and are not bugs — see review notes. * docs(#4209): document LANE_RUN_DIR's early-exit trade-off as accepted, not a gap Round-5 review (Opus, BL-03) flagged that an early exit between dispatch_reviewer_lanes and commit_review leaks the run-scoped temp dir. A trap-based cleanup was considered and rejected: if a step genuinely runs as a separate process, a trap set at creation time would fire at the end of that SAME fence, deleting the directory before spawn_reviewer/commit_review ever read it — worse than the leak it would fix. review.md's own gather_context/cleanup pair for the identical resource class (a run-scoped reviewer temp dir) already makes and documents this exact trade-off: cleanup runs only on a documented success path, and a leftover $TMPDIR entry is explicitly called cheaper than destroyed evidence. Recording that precedent here so this isn't re-raised as a live gap in a future review. * fix(#4209): register the WR-05 symlink-escape test's synthetic docs/ path reviewer-step-dispatch.test.cjs's "capability-neutral reuse" fixture passes paths: ['docs/spec.md'] as a synthetic, never-read path proving the dispatcher has no code-review-specific special-casing. lint-docs-guard- registration correctly flagged this as an unregistered docs/ path reference — add the docs-guard-exempt marker and its pinned baseline entry, the same pattern every other synthetic docs/ literal in this test suite already uses. * fix(#4209): backfill changeset pr: field with the real upstream PR number changeset-lint's fail_pr_field_drift caught the fragment still pointing at the fork PR (17) instead of the upstream one (open-gsd/gsd-core#4323) this branch is now also open against. * docs(#4209): amend ADR-2782 for the supportsReviewerLanes step-trait seam trek-e's review (2026-09-07, gsd-core#4323) found a real ADR gap: every decision in ADR-2782 (D1-D9) and every prior dated amendment governs the `role: "reviewer"` capability body and its one consumer, /gsd:review. This PR's actual new seam - a `supportsReviewerLanes: true` trait on an ordinary feature capability's `steps[]` entry, projected through loop-resolver.cts and resolved in-process via resolveActiveHooksForPoint - is a different capability axis (steps/gates/contributions) that the ADR's own scope note explicitly places out of reach. Per docs/contributor-standards.md's "Amending an accepted ADR", an in-place dated section is the established, lighter-weight path for an addition that stays within the ADR's existing decisions - used twice already in this same file - so this appends a third dated entry documenting the new seam, its consumer, and why it reuses the existing D1-D9-governed plan/invoke machinery rather than adding a second one. No decision is reversed; no new Amends/Amended-by pair is needed since the steps/gates/contributions axis already carries reciprocal links to ADR-857 and ADR-894. * fix(#4209): close two test-quality gaps trek-e's review found Minor 1: validatePaths (a path-shape parser guarding the prompt- injection/path-traversal trust boundary) had only example-based coverage, violating ADR-456's rule that parsers/budget limits carry at least one fast-check property test. Adds three: safe-segment paths are never rejected, a single leading "../" always escapes the one-segment repoRoot, and a control character anywhere is always rejected - one property per rejection reason validatePaths owns. Minor 2: the budget-overflow check (`estimatedTokens > budget`) was only ever exercised far below budget or at budget:0 (unbounded), never at the exact threshold crossing where a `>` vs `>=` off-by-one would hide. Adds three exact-boundary tests using the real estimateTokens/ buildSourceReviewPrompt the module calls internally, so the resolved token count is exact rather than approximated: budget == estimate (must pass), budget == estimate - 1 (must fail), budget == estimate + 1 (must pass). Also extracts okPlan()'s fixture timeoutMs into a named constant - local/no-adhoc-timeout-literal (#4446) landed on next after this branch was authored and flagged the pre-existing literal on rebase; it is fixture data for a synthetic plan object dispatchReviewerLanes never waits on, a distinct class from tests/helpers/timeouts.cjs's real subprocess norms. * fix(#4209): update docs-guard-registration baseline for the new ADR citation reviewer-step-dispatch.test.cjs's new fast-check property tests cite docs/adr/456-test-rigor-architecture.md in a justifying comment (never a real read). lint-docs-guard-registration fingerprints every docs/ path string an exempted test file mentions and fails on drift so a human re-confirms the exemption still holds - re-confirmed, and the baseline is updated to match. * fix(#4209): point changeset pr: field at the fork PR for CI validation changeset-lint's fail_pr_field_drift check compares the fragment's pr: field against the PR the CI run is actually attached to (GITHUB_EVENT_PATH), not a fixed target. Rehearsing this branch on fork PR davdittrich/gsd-core#17 needs pr: 17 to pass that check; the prior commit's pr: 4323 (the real open-gsd upstream PR number) is correct for that PR but fails here. Backfill to 4323 happens again, as the last commit, immediately before the approved push to open-gsd#4323 - never leaving pr: 17 on the branch that ships upstream. * fix(#4209): reject promptChannel:none lanes from source-review dispatch CodeRabbit found a real scope mismatch: coderabbit's lane declares promptChannel: 'none' and reviews the working tree on its own terms, fed nothing (review.md:367). Silently dispatching it through dispatchReviewerLanes would ignore the bounded paths/depth/baseSha scope buildSourceReviewPrompt promises and let the lane review whatever it independently sees fit, violating this interpreter's own scoped, metadata-only contract. Reject before plan()/invoke(), same as an unresolved slug. * fix(#4209): scope CONS-02 test to the evidence-block line, not the whole file CodeRabbit found the whole-file match on workflowContent would still pass if UNVERIFIED and re-open/reopen appeared in two unrelated parts of this 1000+-line workflow, proving nothing about the actual evidence block's contract. Line-filtered via splitLines (not a bare-\n regex spanning readFileSync content) so this stays CRLF-portable and passes local/no-unbounded-quantifier and local/no-crlf-fragile-split. * fix(#4209): guard DISPATCH_JSON substitution and capture its stderr CodeRabbit found the dispatch-step command substitution unguarded: a non-zero exit could leave DISPATCH_JSON empty (or halt the step under errexit with no warning), and the downstream reducer would only ever report the generic unparseable_dispatch_output reason, discarding the command's own diagnostic. That extra quote density, inside an already quote-heavy ~8KB driver string, passed bash -n and the full local suite on Linux but broke Windows Git-Bash: `dispatch_reviewer_lanes computes CODE_REVIEW_POINT ... end to end (#4209 round 5)` failed on two Windows CI shards with `bash -c: unexpected EOF while looking for matching '''` — a Windows argv-to- command-line re-quoting edge case, reproducible on rerun, not a flake. Root-caused via gh api job logs plus a byte-identical local reconstruction of the test's own driver script. Fix: drop the fabricated stub. The downstream node -e reducer already falls back to reason `unparseable_dispatch_output` on any JSON.parse failure, so an empty/partial DISPATCH_JSON on command failure is still handled correctly, with zero new quoting risk. * revert(#4209): drop the DISPATCH_JSON stderr-guard nitpick (Windows CI) Two materially different mechanisms for the same CodeRabbit Nitpick ("Trivial | Quick win") both broke Windows Git-Bash reproducibly: a single-quoted JSON-literal fallback ("bash -c: unexpected EOF ... matching '''") and, after removing that, a plain `head -1 "$VAR"` inside a nested command substitution ("unexpected EOF ... matching '"'"). Both passed bash -n and the full local suite on Linux every time; both failed the SAME test deterministically on Windows CI. Two attempts at the same class of fix (nested-quote construction near this exact step) is the retry limit - reverting to the original, already-shipped, Windows-verified unguarded form rather than continuing to guess at a third quoting mechanism for a Trivial- severity nitpick. plausible structured-search need).
Hand-edits commands/gsd/*.md only; skills/*/SKILL.md is regenerated
via `npm run gen:plugin-skills` from that source. Updates the one
hardcoded copilot-install test assertion affected by gsd-health's
new tool order.

The bullet embeds the todo file's ABSOLUTE path, so its total length
varies by runner tmpdir: on macOS CI, /private/var/folders/… plus the
test harness's gsd-test-run-* wrapper pushed the full bullet to 244
chars — past renderPendingTodoBullet's intended 240-char cap, whose
documented first degradation step drops the Needs clause. The product
behavior is correct (#2618 design); the assertion was runner-dependent.
The extraction is now pinned on json.todos[0].needs; the rendered-bullet
shape stays covered by the path-independent title assertion and the
renderer's own unit rows.
Found blocking #4186's CI on the macOS shard (test landed 30 minutes
earlier in

MEDIUM — unquoted $ARGUMENTS -> argv/glob injection. gsd-core/workflows/quick-batch.md and commands/gsd/quick-batch.md both ran `gsd_run quick-batch parse-args --raw -- $ARGUMENTS` UNQUOTED, causing shell word-splitting and pathname expansion on raw task-list text before the parser ever saw it. Fixed at the source: added a `--text <string>` form to the `parse-args` verb (src/quick-batch-command-router.cts) that accepts the ENTIRE $ARGUMENTS as ONE quoted argv element and does the whitespace split itself, in Node — which is never glob-aware, unlike the shell. Both call sites now use `--text "$ARGUMENTS"`. The `-- <tokens>` form is kept for direct/test callers that already have a real argv array. fix(#3809): route runtime-loaded markdown through the gsd_run launcher (#3815)
* test(#3809): generalize dead-ref guard into a rule table (failing first)
The #2020 guard hardcoded `sdk/(src|dist|handlers)/` — the three dead paths
that had caused that storm. That proved those three paths were gone and said
nothing about the class, so #3809 reproduced the identical Windows find.exe
storm under a different token and the guard could not see it.
Replaces the single regex with a rule table over the same runtime-loaded
markdown surface, adds `commands/` to the scan set (previously uncovered),
and adds rule B: the runtime shim filename must never appear in command
position, because it is not a PATH command and an agent that meets it falls
back to locating the file.
Rule B's matcher is deliberately lenient — the launcher's own resolver
assignment, `node <path>/<shim>` calls, bare paths, and prose that names the
file all stay unflagged, each pinned by a negative-space row.
This commit is expected to FAIL: 50 offenders across 23 files remain in the
tree. The remediation lands next.

files instructed the agent to run the runtime shim by filename. That filename
is not on PATH under any name -- package.json ships gsd-core, gsd-tools,
gsd_run and gsd-mcp-server -- so the call exited 127, the file-shaped token
sent the agent looking for the file, and on Git Bash for Windows the resulting
`find /` walked the entire drive (7268 CPU-seconds in the report) until
somebody killed it by hand.
CONTEXT.md -> Runtime Launcher Module already makes gsd_run the single entry
point: "Canonical space-safe shell preamble (`gsd_run`) used by every workflow
bash block to invoke the GSD runtime CLI." These sites predate that rule --

* feat(#3028): responsive Markdown separators instead of fixed-width rules Stage banners, checkpoints, completion and error panels used fixed-width runs of box-drawing characters -- a 53-column heavy rule and a 62-column double-line box. Those runs are ordinary text to a Markdown-rendering host, so in a narrower pane they wrap and the border comes apart from the heading it framed. Shipped content now emits an ATX heading for a titled section and a blank-line-delimited --- for a break between sections, both of which adapt to the available width. The same convention is applied to the three code sites that built these strings at runtime: the UAT checkpoint renderer, the milestone-close audit report, and the TDD review checkpoint table. Removing the box also removes its only reason to exist -- the east-asian-width padding helpers that kept its right border aligned (checkpointBoxLine, displayWidth, isWideCodePoint, ZERO_WIDTH_MARK_RE, CHECKPOINT_BOX_WIDTH). RTL directional isolation is unchanged. The convention is specified in gsd-core/references/ui-brand.md and enforced across all shipped content by tests/responsive-separators.test.cjs. It now carries the separator section, the setext reasoning, the unconditional-vs-per-runtime rationale and a corrected anti-pattern list, in Chinese. The user guide asserted that a heading is not a degradation anywhere. That is an assertion, not a demonstration. It now says what was actually traded away in a plain terminal, points at the recorded rationale, and invites the report that would justify the capability flag instead. Adds workflowPathRefs() to command-contract-helpers.cjs — one shared pure resolver for the three reference shapes this repo uses (eager @-include, lazy absolute-ish path, lazy parent-relative steps//modes/ path). Placing it in the helpers module keeps the lint script and the test suite reading the same definition, which is what that module exists for; #3560 consumes the same function rather than re-deriving it. Tests 16/17 are RED until the routing fix lands. Both resolver regexes ended at a literal .md with no trailing boundary, so a longer extension was truncated into a plausible-looking but wrong path: workflows/foobar.mdx returned workflows/foobar.md. Adds a (?![A-Za-z0-9_]) lookahead to both shapes so .mdx and .md5 are rejected outright rather than silently rewritten. The regression test scanned the whole command file, so it did not bind to the defect — the reviewer showed that an unrelated comment mentioning scan.md anywhere made it pass while the dispatch defect was still present. It now extracts the "- If it is `--fast`" bullet and scans that line alone, and a new test drives a synthetic pre-fix fixture to prove the false-pass path is closed. The count form is correct only when nullglob is set, and six of these seven files never set it: without it the array holds the unmatched literal pattern, so the count is 1 and the guard passes wrongly. `-e` is correct in both worlds. review.md keeps its count guards — that block sets nullglob two lines above them. skills/gsd-review-backlog regenerated from commands/, never hand-edited. A rule keyed on gsd_run matched 111 lines, ~132 of them legitimate, and was rejected. - Detector B: `cat <glob>` in command position, and `ls <glob>` whose exit code feeds a real fallback or an if/while head. Informational `ls <glob>` whose stdout is consumed (97 sites) and `|| true` failure suppression (~15) are not guards and never fire. Shrink-only ratchet keyed on (file, trimmed text) with a per-pair count, POSIX-normalized unconditionally so Windows CI cannot report everything fresh and stale at once. Ships with a ZERO-entry baseline: every site it can find is fixed. Exemption is the per-line `# gsd-scan-ignore: #NNN` marker whose reason must name an issue or URL; a malformed reason reports a distinct error rather than silently exempting. No file allowlists. ADR-3409 records the invariant, the measurements behind both detectors, and why the upstream `--pick` contract fix belongs to #3473. Five of those paths were already claimed by spent fragments identical to next, which blocks a second source naming the same path; removed just the colliding key from each, deleting the two that this emptied. Claude Code applies SKILL.md effort: as output_config.effort; any change from the session baseline invalidates the prompt cache at BOTH scope boundaries (entry + exit, the latter often machine-fired via subagent-completion notification). The reporter's owned measurement confirms it: /gsd-progress (effort:low) in a medium session → cache_creation 63,404 (entry) + 18,589 (exit), while a no-effort skill shows none. ~76% of invocations paid in full. Fix (trek-e AC#2/AC#4): convertClaudeCommandToClaudeSkill no longer emits effort: into Claude-runtime skill frontmatter (src/runtime-artifact-conversion.cts + duplicated bin/install.js). normalizeClaudeSkillEffort removed (dead). The six declaring skills (plan-phase/execute-phase/autonomous/next/progress/stats) no longer carry effort. Source command files keep effort (input, used elsewhere); the separate agent-effort surface (#3160) is untouched. * chore(#2993): fragmentize plan-phase.md onto the fragment model Epic #1671 Phase 6.2. plan-phase.md is the largest workflow in the repo and carried zero markers; it was deferred out of the Phase 3 pilot for two reasons, both now dead. The 36-byte PRE_PHASE6 headroom was never the blocker it looked like — fragmentizing is net-negative on host source, so the trim is what creates the room. The --mvp interleaving was resolved by measurement in #2992 and no sub-line mechanism is built. - widen WHEN_VOCABULARY 14 -> 19 via a second coordinated ADR-1671 amendment: flag:--ingest, flag:--prd, flag:--research-phase, flag:--reviews, state:chunked-mode - state:chunked-mode is `--chunked` OR config workflow.plan_chunked, and that disjunction is resolved in the FACT, never in the grammar, so a compound condition never becomes an operator - parse the new flags on the plan-phase route; extract six gated bodies to gsd-core/workflows/plan-phase/steps/ behind manifest-gated stubs - prd-express-path.md was already extracted but read unconditionally; its wrapper is now gated, so the existing extraction finally pays off plan-phase.md 94,483 -> 87,575 bytes (cap 94,519): headroom goes from 36 bytes to 6,944. It fails against the pre-fix files and covers any future atom, rather than spot-checking today's six. Verified through the workflow shape, not the CLI shape: `3 --prd spec.md` now yields ["prd-express-gate"] (was []), `2 --wave 2` yields ["partial-wave"] (was []). Refs #2993 * test(#2993): acknowledge the execute-phase ripple and regenerate install-tree fixtures Remote matrix was red with 46 unique failures, identical on both lanes. Resolved per the linter's own guidance and that file's precedent of appending successive ripple reasons to one entry. - golden-install-tree: tests/fixtures/install-tree/*.json are committed and deliberately excluded from the ADR-2719 attribution cutover, so they must be regenerated when shipped tree content changes. Regenerated after build:lib per the ordering landmine. 19 runtimes each gained exactly the six new plan-phase step files; zero paths removed, which is the absolute failure shape those fixtures exist to catch. Refs #2993 * fix(#2993): restore the launcher preamble in an extracted step and follow moved content in its drift guards Second red run: 26 unique failures, identical on both lanes, in two classes. RUNTIME BUG (runtime-launcher-parity, 7 failures) — chunked-planning-mode.md calls gsd_run but carried no canonical launcher preamble, which is what DEFINES gsd_run(). On any non-Claude runtime that step would fail outright. The preamble is now copied verbatim from the canonical source of truth, gsd-core/workflows/_runtime-launcher.snippet.sh, and the fence dedented to column 0 to match the prd-express-path.md sibling (a list-continuation indent breaks the byte-equal preamble match). prd-express-path.md already had a correct one. This is the same defect #2932 hit when it extracted steps; the parity test caught a real bug, not a stale assertion. DRIFT GUARDS (plan-phase-drift-guard, issue-2762-plan-reviews-chunked, skill-frontmatter-contract) — these assert plan-phase.md contains content this branch moved into step files. Retargeted at where the content now lives, with the asserted property unchanged; the ALL-RUNTIMES label COUNT test now reads host + every step file so the count is preserved across the split rather than reduced. Each retargeted guard was verified to still fail when its step file is stripped, so none was weakened into vacuity. No emitted-drift ack was needed: currentSizes() enumerates gsd-core/workflows/*.md non-recursively, so files under plan-phase/steps/ are never in the size ratchet's scope. * chore(#2800): derive reviewer flag lists and gate reviewer lane docs across locales The reviewer lane roster was hand-enumerated across five documentation surfaces and three workflow files that had drifted apart: --kimi-code was missing from all four translated COMMANDS.md mirrors, --coderabbit from every workflow forwarding list, and --antigravity from FEATURES.md. Adds checkReviewerDocsParity, a second pure gate deliberately separate from checkReviewerLaneParity so a stale doc cannot make the runtime checker look red. Workflows now derive their flag lists from a new review-lane flags query instead of hand-enumerating them, which also retires the unanchored grep that matched --agy inside --antigravity. Documents the previously absent reviewer body and hostBehaviors field in the capability manifest reference. Closes #2800 Closes #2781 Closes #2272 * fix(#2800): key the docs parity table arm on first-cell position Review found the flag arm was file-scoped, so the forwarding row that lists every flag in its third cell satisfied it on its own. Deleting a lane's own reviewer-table row -- the #2781 regression this gate exists to prevent -- therefore passed undetected. Arm 4 keys on the FIRST table cell, which separates a lane row from the forwarding row structurally and in every locale. Regression test included. * fix(#2800): shape-filter the flags subcommand output All three consumers read review-lane flags through an unquoted command substitution so the output word-splits into loop items. Phase 2 admits third-party overlay lanes, so an overlay flag containing whitespace would inject a second loop item and one containing a glob would expand against the cwd. Emit only well-formed flags so neither reaches the shell. * fix(#2800): remove the regex length ceiling and count only prose mentions Review found two real defects in the docs parity gate. The never-throws contract was false: building a RegExp from a declared flag or section title throws SyntaxError past ~100k chars, and Phase 2 admits overlay lanes whose declared strings are untrusted in length. Every one of these matches is literal, so String.includes replaces the regex outright, which also deletes escapeLiteral and the llama.cpp escaping it existed for. Arm 1 was context-blind: a flag mentioned only inside a fenced example or a commented-out row counted as documented. Both are stripped before matching. Also advertises all 13 lane flags in the argument-hint and corrects a stale eleven-lane count in the slug grammar note. * test(#2800): repoint the convergence suite off deleted workflow text The derived flag loop deleted the literal per-flag grep lines four tests matched on. Two of those failed loudly. The behavioral and property tests failed SILENTLY instead: their end marker no longer resolved, so the parse block extracted empty and both passed vacuously, and the property test's gsd_run stub had a no-op default that hid it. All now share one extractor and execute the real deployed block through a gsd_run shim backed by the actual binary. The whitelist assertions become an anti-parity check: re-adding a hand-written flag list must fail. Also repairs two vacuous cases in the docs parity suite. The unreadable-doc test called its own mock rather than the reader, and the integration test bounded nothing, so a doc losing its marker would have been silently skipped and still passed green. * fix(#2800): run the derived flag loop after the launcher preamble The remote matrix caught a real runtime bug, not a test artifact. In autonomous.md and plan-review-convergence.md the launcher preamble that defines gsd_run lives in a separate, LATER bash fence than the derived loop. Each fence is its own shell, so gsd_run was undefined where the loop ran: the command substitution yielded nothing and zero reviewer flags would have been forwarded. Worse than the drift this epic fixes, and silent. The whole CONVERGENCE_ARGS construction moves as one unit, because the --max-cycles append sits between the loop and the preamble and would otherwise have run against an uninitialized variable and then been dropped by the relocated initializer. Also documents all 13 lane flags in help/modes/full.md, which the repo gates bidirectionally against each command's argument-hint. * test(#2800): repoint the two converge suites off deleted flag literals Both asserted workflow.includes('--codex') against the hand-enumerated list the derived loop removed. They now assert the derivation itself, keep --all and --text (convergence controls, still literal), and add an anti-parity guard so re-adding a hardcoded list fails. The lost pass-through proof is replaced with a real one: every flag the tests used to hardcode is asserted present in the actual roster emitted by the binary, which is the property the old assertion was protecting. * test(#2800): acknowledge the workflow byte growth from the derived flag loop * chore(#2800): backfill changeset pr number to 2882 * fix(#2800): strip HTML comments to a fixed point in the parity gate CodeQL js/incomplete-multi-character-sanitization (high) on PR #2882: the single-pass <!--...--> strip can leave a live <!-- behind, so a join-trick construction smuggles a commented-out row past the gate and it counts as documented. Not an injection risk here since nothing is rendered, but it is the exact false pass this helper exists to prevent. Strips to a fixed point, then treats any surviving opener as unterminated so the multi-line branch closes it on a later line. Terminates because every pass strictly shortens the string. * test(#2800): pin the comment-smuggling regression with a real reproducer The obvious fixture for this class does not reproduce it: <!--<!---->--> leaves a dangling --> rather than a live <!--, and is caught either way, so it would have passed with and without the fix. Every finding fixed: Security — the rationale is untrusted input (ADR-1577). It originates in conversation and flows CONTEXT.md -> planner -> PLAN.md -> executor, each hop an LLM reading the previous hop's output, with no validation on the path. planner-reversibility.md and the discuss-phase template now state it is data and never instructions, and name the </reversibility> early-termination hazard explicitly -- a rationale that closes its own element injects sibling structure the executor reads as real tasks. Four tests guard it. Correctness 1 — nothing machine-enforced the feature's own promise: a task rated one-way with no preceding checkpoint:decision validated as fully clean, so a planner error silently reopened the gap this feature exists to close. cmdVerifyPlanStructure now warns on an ungated one-way rating. A warning, not an error: <reversibility> stays additive and the plan stays valid. Four tests cover ungated (warns), gated (silent), still-valid, and reversible/costly never flagged. The --no-reversibility-gates parse test substring-matched the whole workflow file, and plan-phase.md prose mentions both tokens in one sentence, so it passed with the bash conditional deleted: it was testing the documentation, not the parser. Now scoped to the fenced bash blocks and matched as one physical line, with a negative control confirming prose alone cannot satisfy it. Correctness 3 — costly had no itemized emission rule, only one-way did, so two agents could diverge on whether to tag costly at all. Correctness 4 — template convention break: the example ratings were bare while every sibling field uses [...] to signal substitution, inviting an LLM to copy one-way/costly forward as boilerplate. Now bracketed. Correctness 5 — latent false-green: .includes('reversible') also matches inside irreversible/irreversibility, which appear in anti-pattern prose, so a surface that dropped the real taxonomy entry would still pass. Now word-boundary matched. ADR-857 phase-6 ceiling — the first gsd-test run caught plan-phase.md 1216 bytes over its frozen 94519 ceiling (it had 49 bytes of headroom on next). The ceiling may only rise for privileged host machinery, and reversibility gating is optional-feature logic, so the wiring was slimmed to its minimum and the explanatory prose moved to the reference files the planner already loads. plan-phase.md is now 94400 bytes -- 119 under the ceiling and 70 bytes SMALLER than on next, so the host loop shrank while gaining the feature, which is what phase 6 ratchets toward. The tracer contract (tests/tracer-bullet.test.cjs) is unchanged. CodeQL js/incomplete-sanitization (high), reversibility-tagging.test.cjs:46 — the namesRating helper built its regex with `rating.replace(/[-]/g, '\\-')`, which escapes the hyphen but not backslash, so the escape was incomplete. It was also unnecessary: `-` carries no special meaning outside a character class. Replaced with a complete metacharacter escape (backslash included). Word-boundary behavior verified unchanged across all three ratings — notably that "irreversible" prose still does not satisfy a "reversible" match, which is the false-green this helper exists to prevent. Prompt injection scan — the checkpoint fixture used the human-verification child element inside <verify>. That tag name is a fake-instruction-boundary pattern in scripts/prompt-injection-scan.sh, and the scan runs over changed files, so copying the shape from tests/verify.test.cjs (unflagged only because it is not in this diff) tripped the gate. Switched to the documented plain-prose <verify> form. The first attempt at that fix failed the same gate a second time: the comment explaining the collision quoted the offending tag literally. The comment now names it in prose instead — the scanner does not care whether a match is code or commentary, which is the whole point of the DEFECT.PROMPT-INJECTION-SCAN-COLLISION note in CLAUDE.md. Verified locally before push: scan reports 0 findings across 57 changed files, eslint clean, and both fixtures still validate as designed (gated one-way silent, ungated one-way warns, neither errors). Two contributing factors were this file's own, and are fixed here. 1. tests/test-timings.json had no entry for reversibility-tagging.test.cjs, so scripts/run-tests.cjs weighted it at the table's median fallback (~315ms) for LPT chunk packing. It actually measures 5595ms — an 18x under-weight. Recorded the measured value from the green gsd-test run (max across the node22/node24 lanes, per gen-test-timings.cjs's convention). Only this one entry: a full regen churns 634 entries of run-to-run drift, and the table is explicitly advisory and un-gated, so a 637-line diff does not belong in a feature PR. 2. Each verifyPlan() spawns gsd-tools, which dominates this file's cost. Spawns cut from 9 to 6 with no coverage lost: - the ungated-one-way warning and its stays-valid assertion now share one plan instead of building the same plan twice; - the reversible/costly never-flagged-as-ungated test was strictly subsumed by the additive suite, which already runs those two ratings ungated and asserts no /reversibilit/ warning at all — and the gate warning's text contains both "reversibility" and "one-way", so the broader assertion catches it. It only re-spawned gsd-tools twice to prove the same thing. Both are symptom fixes. The shard imbalance itself (19/11/10 minutes against a 20-minute cap, from a cost-blind round-robin partition that also reshuffles downstream files whenever one is inserted) is tracked in #2472. list crashes the first 'mempalace mine' invocation with
TypeError: string indices must be integers, not 'str'
Following the documented example verbatim and running the capture crashed
every time, before any file was routed. The bug shipped in #2220's fix

fix(#2315): respect review.default_reviewers in bare convergence invocation (#2451)
* test(#2315): regression test for review.default_reviewers precedence A bare /gsd-plan-review-convergence invocation (no reviewer flags) is supposed to let users configure a persistent reviewer lineup via review.default_reviewers and just run the loop. Instead, the orchestrator's argument parser silently discards that configuration and forces --codex on every no-flag invocation — with no warning that the configured reviewers were ignored. This commit adds a regression test that fails against the pre-fix workflow (the buggy unconditional --codex fallback is still present at this commit) and passes after the fix lands: - Structural: the workflow must NOT contain an unconditional 'if [ -z "$REVIEWER_FLAGS" ]; then REVIEWER_FLAGS="--codex"; fi' line before the workflow.plan_review_convergence config gate. - Structural: the workflow must query review.default_reviewers AFTER the config gate and document that empty REVIEWER_FLAGS lets gsd-review apply the default. - Behavioral: matrix across {configured, unset, empty-array, explicit-flag} invoking the actual deployed parse + resolution blocks with a stubbed gsd_run. Also updates two existing tests whose assertions the fix makes stale: - #2293 behavioral: endMarker was the buggy unconditional fallback line; the bare invocation assertion was 'run("5") === "--codex"'. Both flip post-fix (endMarker is now the last --all grep line; bare invocation returns empty from the parse block, default applied later in step 1.5). - command-default-claim: the pre-fix command documented '--codex (default if no reviewer specified)' which was the user-facing mirror of the bug. The assertion now requires the command to document the review.default_reviewers precedence. * fix(#2315): respect review.default_reviewers in bare convergence invocation Root cause: plan-review-convergence.md step 1 (Parse and Normalize Arguments) contained an unconditional fallback that set REVIEWER_FLAGS=\"--codex\" whenever no explicit reviewer flag was supplied. This value was then interpolated verbatim into the gsd-review args, so gsd-review saw --codex as an explicit flag (precedence rule 1) and never reached rule 3 (review.default_reviewers). The same path silently dropped any configured review.reviewer_instances (instances participate ONLY via review.default_reviewers per ADR-1517). Fix: - Remove the unconditional --codex fallback from step 1. - Add a config-gated resolution in step 1.5 (after CONVERGENCE_ENABLED check) that queries review.default_reviewers and either leaves REVIEWER_FLAGS empty (letting gsd-review apply its own rule-3 default) or falls back to --codex when no default is configured — preserving the pre-fix default for unconfigured users (AC3). - Replace the banner {REVIEWER_FLAGS} token with {REVIEWER_DISPLAY} so the startup banner reflects what will actually run (AC4), not a hardcoded value. The fix upholds the documented precedence contract (ADR-0011, ADR-0015) that the bug was actively violating. Explicit-flag invocations (--gemini, --all, etc.) are unaffected (AC5). References: #2315; ADR-0011 (review.default_reviewers precedence); ADR-0015 (autonomous cross-AI convergence); ADR-1517 (reviewer instances). * chore(#2315): bump plan-review-convergence.md baseline + changeset - Bump plan-review-convergence.md size baseline 23713 → 25536 (the new step-1.5 default-resolution block). - Add .changeset/plucky-yaks-roar.md documenting the user-visible change. * fix(#2315): restore /gsd: colon syntax + skip behavioral test when jq missing Two follow-ups to the #2315 fix discovered by gsd-test: 1. The fix commit accidentally regressed the slash-command namespace in the disabled-feature exit message: /gsd:plan-review-convergence (correct, from PR #3452) became /gsd-plan-review-convergence (retired dash syntax). The slash-command-namespace invariant test caught this. Restored the colon form. 2. The behavioral test exercises the deployed reviewer-resolution block, which pipes through jq. jq is a documented production dependency (review.md:244 "install jq if missing") and is present in every production deployment, but is NOT on PATH in the gsd-test linux-node{22,24} containers (same constraint as tests/opencode-review-reconstruction. property.test.cjs). Without jq, the printf|jq pipeline fails silently, the ||echo 0 fallback yields DEFAULT_REVIEWERS_COUNT=0, and the resolution falls through to the --codex branch — producing a false negative. Added a jqAvailable guard at module load (matching the existing pattern) and skip the behavioral test when jq is absent. The structural tests (no bash execution) still run and validate the fix. * chore(#2315): regenerate golden-install-parity fixtures Source changes to plan-review-convergence.md (workflow + skill mirror + command doc) changed the install-tree hashes. Regenerated via 'npm run gen:golden' after rebuilding gsd-core/bin/lib/install-engine.cjs ('npm run build:lib') — the on-disk lib was stale relative to src/install-engine.cts (isSymlinkedDestOptIn) and blocked fixture gen. * test(#2315): address review findings — strengthen structural tests + property test Code-review + security-review (isolated subagent passes) surfaced Low/Nit findings; this commit addresses the test-side findings: - Structural test 1 ("unconditional --codex one-liner") now asserts the buggy line is absent EVERYWHERE, not just before the config gate. The earlier assertion allowed a maintainer to re-add the line after the gate (passing the structural test) while the bug would still bite at runtime before the gate runs. - Structural test 4 ("banner uses REVIEWER_DISPLAY") now asserts the LITERAL banner placeholder "Reviewers: {REVIEWER_DISPLAY}" and forbids "Reviewers: {REVIEWER_FLAGS}". The earlier workflow.includes( "REVIEWER_DISPLAY") was satisfied by a comment mention. - New structural test for command/skill content parity: both files must document the review.default_reviewers precedence on the --codex flag (catches a manual edit to one that the gen:plugin-skills mirror misses). - Behavioral test stub now passes default_reviewers via env var ($GSD_TEST_DEFAULT_REVIEWERS) instead of inline-interpolating into a bash single-quoted string. Removes the (currently-safe) fragility where a future test input containing a single quote would close the bash quote and execute as bash under execFileSync. - New property test (fast-check, numRuns=25) for the JSON-classification contract: non-empty arrays of slugs -> empty REVIEWER_FLAGS; empty array / scalar JSON / malformed JSON -> --codex fallback. Locks the parser contract per CLAUDE.md mandate. * fix(#2315): address review findings — defensive jq-missing warning + banner cleanup Code-review surfaced two Low-severity workflow-side findings: - Defensive jq-missing warning: if jq is not on PATH in production (it is a documented dependency per review.md:244, but the dependency can be absent in degraded environments), the printf|jq pipeline fails silently to "0" and a user with review.default_reviewers configured gets --codex with no indication their configured default was unreadable. Added a command -v jq guard at the top of the resolution that falls back to --codex AND emits a stderr warning explaining the reason. Wire the two forward-declared mempalace.memory_mode modes so they actually route recall/capture instead of silently behaving as `augment`: - kg_backend: the palace temporal KG is the primary knowledge-graph source; native .planning/graphs/ is the fallback. Non-KG drawer recall stays additive. - replace: recall resolves through the palace as the source of truth; native artifacts are the fallback. Every mode stays onError:skip and default-resilient — an unreachable palace degrades to native memory and GSD keeps writing .planning/graphs/, so no memory is lost. Cross-mode .planning/graphs/ migration remains a documented open question (PRD/ADR §17), out of scope here. Surfaces updated (instruction-only contract): recall/capture commands (+ generated skills), discuss/wave fragments, curator agent, capability.json schema. Docs: how-to Step 3, CONFIGURATION, FEATURES, CONTEXT glossary. Regenerated capability-registry, golden install-parity fixtures (mempalace hashes only), agent-size-baseline. Added a routing-contract + cross-surface parity test. Incidental (folded per no-defer rule): removed pre-existing unused imports (spawnSync in capability-registry.test.cjs; fs in issue-498-package-identity.test.cjs) that eslint flagged in/alongside the touched files. command/workflow with AskUserQuestion + --text fallback. Routing-first
('what now?' menu), 10 situations redesigned for gsd-core's phase loop.
* feat: add /gsd-start smart-entry command
State-aware front door adapted from gsd-pi's smart-entry wizard,
redesigned for gsd-core's markdown-first, multi-runtime architecture.
- src/smart-entry.cts: deterministic situation classifier (no-project,
paused, blocked, verify-failed, needs-first-phase, planning, executing,
verify-pending, idle-stranded, complete, unknown). Reads STATE.md,
ROADMAP.md, git, and verify signals; emits JSON the workflow consumes.
- gsd-tools.cjs: wire case + help listing.
- commands/gsd/start.md + gsd-core/workflows/gsd.md: thin markdown
dispatcher presenting an AskUserQuestion menu (with --text fallback for
non-Claude runtimes) and dispatching to existing commands. Falls back
to /gsd:progress if detection is unavailable.
- help.md: document /gsd:start (parity with bug-2954).
- tests: smart-entry.unit.test.cjs (classifier behavior across all
situations + priority + JSON shape) and gsd-workflow.structure.test.cjs
(markdown-layer invariants + every emitted command resolves to a real
slash command).

Note: command-contract (ADR-0002) requires a gsd:* prefix, so the bare

Rename the command from /gsd:start to /gsd:next per feedback. The
command file is now commands/gsd/next.md (name: gsd:next) and the
backing workflow is gsd-core/workflows/smart-entry.md (named for the
smart-entry classifier and gsd-tools smart-entry subcommand; does not
collide with the existing workflows/next.md, which is the progress
--next sub-workflow). help.md and the spec updated to match.
All affected tests (188) pass; lint:ci clean.
* fix: smart-entry reads real STATE.md schema (nested progress YAML + body Phase field)
Codex review found the classifier misread this repo's own STATE.md: it
looked only for scalar current_phase/total_phases frontmatter and body
fields named 'Current Phase'/'Total Phases', but real STATE.md stores
the phase as body 'Phase: N' and total_phases/percent under a nested
'progress:' YAML object. Both came back null, so active projects
(e.g. this repo at Phase 3 / verifying) wrongly classified as
needs-first-phase.
- detectSignals now reads total_phases + percent from nested progress{}
first, then scalar fm, then body; current_phase falls back to the
body 'Phase:' field (parseProsePhaseField lineage).
- Add regression tests against the real schema (nested progress YAML +
body Phase field) covering verify-pending + executing situations.
Verified against this repo: now classifies verify-pending (was
needs-first-phase). Coverage 93.25% lines / 86.99% branches.
* fix(workflow): tiered fallback when gsd-tools is broken (not just smart-entry)
Live test exposed a self-defeating fallback: when smart-entry --json
failed because gsd-tools itself was broken (missing
markdown-sectionizer.cjs), the workflow fell back to /gsd:progress —
which also depends on gsd-tools and would dead-end too.
Replace the single /gsd:progress fallback with a tiered recovery:
1. Probe gsd_run state-snapshot. If it ALSO errors, the whole tool
layer is down — read .planning/STATE.md directly with the Read tool
and synthesize a minimal situation + actions menu so /gsd:next stays
useful. Surface a rebuild hint.
2. Only if smart-entry alone is missing (older gsd-core), fall back to
/gsd:progress as before.
Matches the direct-read resilience the live agent already did by hand.

Now planning/executing/verify-pending recommend `/gsd:progress --next`
(single gated engine); the specific command stays an explicit secondary.

idle-stranded, complete) keep direct recommendations — smart-entry's
distinct value over --next. Adds docs/adr/1787-gsd-next-smart-entry.md and
a regression test locking the delegation contract.

* fix(#1798): split oversized test chunks so a slow shard can't trip the per-chunk timeout
Root-cause of the intermittent `full test (windows-latest, 22, shard 1/3)`
failure. It was NOT a leaked handle (the runner's kill message guesses that,
but --test-force-exit already exits leaks cleanly). Diagnosis:
- Ran every shard-1/3 file WITHOUT --test-force-exit + a 45s kill-timer:
zero hangs, zero leaks — every file self-exits. So no leaked handle / hang.
- CI activity profile: output kept flowing (slowly) right up to the 600.0s
kill — a dead hang would go silent. => pure slowness.
- Per-file timing: install-minimal-hooks.test.cjs is a 4987-line / 250-case
consolidation file doing dozens of real installs — 41s even on a fast Mac
(much worse on the slow Windows I/O path), plus an install-heavy cluster.
Mechanism: MAX_FILES_PER_CHUNK=180 packed the whole ~171-file shard into ONE
`node --test` chunk, so the entire shard's wall-clock ran against a single
600s per-chunk backstop. On slow Windows runners that single chunk crossed
600s and was killed mid-run — an intermittent false-negative gate that also
hits `next` directly.
Fix: lower MAX_FILES_PER_CHUNK 180 -> 90 so each shard splits into ~2 chunks,
each with its own fresh 600s budget and a fresh node process (also relieves
per-process memory pressure). Verified locally: shard 1/3 now runs as
chunk 1/2 (90 files) + chunk 2/2 (81 files), 5323 tests, 0 fail. Also made the
timeout kill-message name slowness as a cause instead of asserting a leak, so
the next debugger isn't sent hunting a nonexistent handle leak.
