Lex Christopherson 46d63991dd fix: yolo mode now skips confirmation gates in plan-phase

Restructured confirm_breakdown and decision_gate steps to check
mode FIRST with explicit skip instructions, preventing Claude from
reading through and executing the prompting path regardless of config.
