Lex Christopherson f3e0e69ce6 refactor(audit-milestone): command is orchestrator, not subagent ...

Subagents can't spawn subagents. The command itself orchestrates:
- Wave 1: Spawn gsd-verifier per phase (parallel)
- Wave 2: Spawn gsd-integration-checker
- Aggregate results into MILESTONE-AUDIT.md

Remove gsd-milestone-auditor agent (command does this now).
