feat(06-04): implement dial-time SSRF-guarded Fetch
- AllowHosts exact/dotted-suffix and PublicOnly modes, https-only - Private/reserved IP check at net.Dialer.Control, no redirects - Streaming io.LimitReader byte cap and typed failure reasons
