94d9344cdd docs(19.3): revise gap closure plan 06 after checker feedback 
Task 1 tests every refusal status routing.md names (403 and the
Turnstile 422 and 503 added). Task 4 ends with a committed progress
record and the plan gains a resume rule, so a context reset after
Task 4 continues at Task 5 without a partial SUMMARY.

e37ccbeacb docs(18.1): revise plans per checker feedback 
Mark RESEARCH open questions and premise shifts resolved by D-15..D-22 and flag the superseded recommendations (S-6, S-7, Pattern 9, five-plan split). Split oversized tasks inside plans 01, 02, 04 and 06 (plan count unchanged per D-22) and record the per-plan trade-off for plans 04 and 06.
