feat(11-02): add parallelization frontmatter to write_phase_prompt

- Include parallelizable, depends_on, files_exclusive in frontmatter
- Rules for parallelizable field determination
- Parallel-aware context section guidance
- Avoid reflexive SUMMARY references when parallel
