docs: add recommended permissions mode to README

Addresses #16 - documents that GSD is designed for
--dangerously-skip-permissions mode, with fallback
settings.json config for users who prefer granular control.

GSD is designed for frictionless automation. Run Claude Code with:

This is how GSD is intended to be used — stopping to approve `date` and `git commit` 50 times defeats the purpose.
