a9a7a328e6 refactor: hard-fork GSD -> MSD (Make Software Done)
Mechanical rename produced by scripts/msd-rename.cjs: gsd/Gsd/GSD -> msd/Msd/MSD
across contents and paths, upstream package/repo coordinates -> @golem15/msd-core
and golem15com/msd-core. Deep links into upstream history, sibling upstream
packages, the GSD-2 import feature, CHANGELOG.md and .changeset/ are kept as-is.

Hand edits on top: MSD block-letter banner and logos, LICENSE copyright line,
package/plugin identity, regenerated lockfile, install-tree fixtures, derived
registries and benchmark baseline; migration checksum baseline re-locked
(MSD keeps its own install state, so no install had applied the old sums);
sort-order and regex-escaped expectations in tests adjusted.

`must_haves.artifacts` entries are read with `safeReadFile`, which rethrows every
errno except ENOENT. A listed path that is a directory therefore threw EISDIR out
of the per-artifact loop: `query verify.artifacts` printed

    Error: EISDIR: illegal operation on a directory, read

and reported NOTHING — not the offending entry, and not the plan's other,
perfectly checkable artifacts. One directory entry disabled the whole plan's
check. Reproduced against a real plan before the fix, and after.

A directory is now reported as that entry's own failure, with an issue distinct
from `File not found` (the path did resolve; it simply is not the thing an
artifact entry can be checked against), and every other artifact in the plan is
still checked and reported independently. Anything else the stat or read throws
becomes that entry's failure too, carrying its errno, rather than discarding the
run — a check that disappears is worse than one that fails, because a failure is
visible.

Verifying directories properly — matching `contains:`/`min_lines:`/`exports:`
across the files inside one — is a feature decision and deliberately not made
here, per the issue's stated scope. Authoring-time rejection of a directory path
is likewise left alone: the brief raises it as a separate question, and the
runtime fix does not depend on it.

Also, found in pre-PR review and pre-existing: `safeReadFile(...) || ''` turned a
post-stat ENOENT into empty content, so an artifact declaring only
`path`/`provides` had no criterion left to fail and passed, having checked
nothing. A null read now reports that instead of inheriting a pass. It can only
turn a false pass into a failure.

Verified: reverting src/verify.cts to the merge-base turns both new rows red with
the exact EISDIR message; lint:ci exit 0; full suite 24/24 chunks, 37,280 tests,
0 failures; tests/verify.test.cjs 219/219.

Review findings from #4735.

Major — the two error branches this PR adds were untested, and the PR body
claimed the mid-check ENOENT window was "not deterministically reproducible
through the CLI seam these tests drive." That was wrong: ADR-3574 records this
repo's convention for exactly this — inject filesystem failures by
monkeypatching the fs method, never by chmod or mode-bit tricks, which root
bypasses and yields a test that passes with zero coverage in root Docker and CI.

Both branches are now pinned that way. The injection runs in the CHILD via
NODE_OPTIONS=--require, because `output()` writes fd 1 directly
(`writeAllSync(1, …)`, io.cjs) rather than through console.log, so an in-process
call cannot have its JSON captured. The preload patches the child's own module
objects, which the compiled code reads at call time.

  - a file that disappears between stat and read now fails as that entry rather
    than passing on empty content
  - a non-ENOENT errno (EACCES) is reported as that entry's failure, carrying its
    code, so an operator can tell a permissions problem from an I/O one

The injection matches the target by path SUFFIX, not string equality: the first
cut compared absolute paths, and a /tmp vs /private/tmp prefix difference
silently disarmed it — the test passed while asserting nothing. A disarmed
injection test is worse than no test, so the reason is recorded at the call site.

Nit — the comment above the try block said the guard "covers what the stat and
read below actually throw", which reads as if the min_lines/contains/exports
checks inside the same block were deliberately guarded too. They are pure string
operations and cannot throw; the comment now says so rather than implying a
guarantee it does not make.

Verified: reverting src/verify.cts to the merge-base turns all three #4685 rows
red — the directory row and both new ones; lint:ci exit 0; full suite 24/24
chunks, 37,389 tests, 0 failures; tests/verify.test.cjs 221/221.

39db16a653 fix(#4678): resolve :LINE citations against their underlying files (#4719)
* fix(#4678): check :LINE citations instead of dropping or misreporting them

cmdVerifyReferences had two opposite failures on citations carrying a
trailing line suffix. The backtick extractor anchored the closing
backtick right after the extension, so `src/foo.ts:42` matched neither
regex and was silently dropped -- an all-line-numbered document
reported {valid:true, found:0, missing:[], total:0}, indistinguishable
from one with no citations. The @-extractor treated ':' as a legal path
character, so @src/foo.ts:42 was probed with the suffix glued on and a
resolvable file was reported missing.

Strip the :N / :N-M suffix (stripLineSuffix) before existsSync in both
loops -- for filesystem resolution only; found/missing keep reporting
the original citation text -- and let the backtick regex accept the
optional suffix so those citations are counted at all. URL skip,
template-placeholder skip, dedup, ~/ expansion and the output contract
are unchanged. Whether a line number past EOF counts as missing is an
open design question and stays out of scope.

The fragment shipped as the documented pr: 0 placeholder; the merge
gate requires pr > 0 before a fragment can land, so backfill 4719 now
that the PR number exists.

Add scanQuantitativeCriteria, the third plan-discipline scanner in the
cmdVerifyPlanStructure family (#429, #968). It judges criteria text in

list of shapes proven to be traps at HEAD: exact grep -c counts (R1),
bulk all-N observed-failing claims (R2), unquoted $VAR in command

An all-string or path-less must_haves.artifacts / key_links block is
item-by-item skipped, leaving zero checked results, yet the pass verdict
was computed as `passed === results.length` (0 === 0), so all_passed /
all_verified read true with status valid and exit 0: a silent false GREEN
over zero acceptance evidence.

Add a positive-evidence floor (results.length > 0) to both verdicts,
mirroring the no-vacuous-pass rule at src/uat-predicate.cts. A well-formed
block, the fully-empty-block error, the parser's string tolerance, and
key-links pending (#1202) semantics are all unchanged.

Governing: ADR-3473 section 8 / 37C (absence, emptiness and failure must
not encode as success) and Decision 3 (failure is a value).

  surface. The prior comment overclaimed symmetry with the artifacts side.
- Add a mixed-block regression test per verb (one bare-string prose bullet + one
  well-formed entry): the string is skipped, results.length === 1 > 0, and the
  verdict follows the single real entry — pinning that the floor does not
  over-reject a partial block.
- Tighten the changeset wording to match (all-bare-string, not "no path:/from: key").

6b7df61938 enhance(#3881): one YAML parser — vendored js-yaml replaces the hand-rolled dialect (#3888)
* docs(#3881): answer §8.1's open question and correct three wrong premises

ADR-3473 §8.1 carries a blocking open question with a forcing function: it must
be answered before any implementation PR for the rule opens. Answered here as (a),
a string-coercing adapter, with the measurement that settles it.

The sequencing note bet that §8.8's schema would make (b) tractable. Measured
against merged reality it does not: only 33 of extractFrontmatter's 78 non-test
call sites read STATE.md, and two of the five compensating mechanisms §8.1 lists
survive real types, leaving ~31 lines across 3 call sites as the actual prize.

Also corrects three claims verified false while answering it. §8.1's justifying
sentence names #3349 and #3360 as defects a real parser would fix; both are
already fixed on next, confirmed by executing the compiled parser rather than
reading it. The guard roster calls lint-frontmatter-scalar-broad-grep.cjs an
expected casualty of this rule, but it guards shell grep idioms in workflow bash
fences and never touches our parser. The same roster calls lint-vendored-deps.cjs
reusable as-is; it is hardcoded to re2js throughout.

The last two were caught by applying the rule this amendment records -- a factual
claim in this ADR is a hypothesis until the implementing phase executes it -- on
its first use.

An adversarial pass on the Phase 4 design established by execution that
extractFrontmatter is not a YAML parser but a line-oriented scanner whose output
is a function of raw source text. Four spellings of the same value collapse to
one js-yaml tree but produce four distinct legacy strings, one of them mangled.
No adapter over a tree can choose among outputs the tree does not distinguish,
so fork (a) -- keep a string-coercing adapter so the existing contract holds --
cannot be built. For any document with a non-scalar value, (a) collapses into
(b); about 26 percent of frontmatter-carrying documents have one.

Also records three design defects and one new attack surface, all confirmed by
execution: catching a parse failure and returning {} would delete the frontmatter
block on the next write at eight call sites that conflate empty with unparseable;
an empty value yields null where legacy yields {}, and reconstructFrontmatter
omits null-valued keys, so the shipped state template's empty progress key would
vanish; the #1882 truncation probe is parseYamlRegion itself rather than a
pre-parse heuristic, so it cannot both stay unchanged and survive that deletion;
and FAILSAFE_SCHEMA still resolves aliases, expanding seven lines to 22.8 MB.

The rule is not deferred. The measurement is the deliverable and the re-scoping
is recorded as an open question with a forcing function, per section 8's own rule.

Packaging step for ADR-3473 §8.1: makes js-yaml available to gsd-core/bin/** without promoting it out of devDependencies (promoting broke every installed tree, #3496).

gsd-core/bin/lib/vendor/js-yaml.cjs is a verbatim copy of node_modules/js-yaml/dist/js-yaml.js (the self-contained UMD dist bundle, not index.js), exposing load/dump/FAILSAFE_SCHEMA/YAMLException with zero require() calls of its own.

src/vendor/js-yaml.d.cts is hand-authored, not copied, because js-yaml ships no upstream .d.ts and @types/js-yaml is not installed. It is deliberately narrow, declaring only the four symbols in use, so anchors/aliases/custom types/loadAll are unreachable from typed code -- a compile-time enforcement of ADR-3473 §8.1's refusal to expand alias resolution for security reasons. Because it has no upstream counterpart it is excluded from the byte-compare.

scripts/lint-vendored-deps.cjs is refactored from a script hardcoded to re2js into a table-driven VENDORED manifest (one row per package: upstream/vendored .cjs paths, optional .d.cts paths, twin kind upstream-verbatim vs hand-authored) so a second vendored package does not require a second hardcoded check block, per ADR-3473 §8.3 'one implementation per rule'. The four existing re2js checks (vendored .cjs vs node_modules, vendored .d.cts vs node_modules, src/vendor twin vs bin-side twin, devDependency version pin vs installed version) are preserved unchanged; verified pass/fail identical before and after the refactor, and the guard's ability to fail was re-proven with a deliberate one-byte append to both re2js.cjs and js-yaml.cjs, then restored.

docs/INVENTORY.md and docs/INVENTORY-MANIFEST.json (via gen-inventory-manifest.cjs --write, run after build:lib) register vendor/js-yaml.cjs. gsd-core/bin/lib/vendor/README.md documents both vendored packages and the two twin kinds.

ADR-3473 §8.1: extractFrontmatter's read path is no longer a hand-rolled
line scanner. parseYamlRegion, escapeDoubleQuoted, unescapeDoubleQuoted and
parseQuotedScalar are deleted (not patched); parsing now goes through the
vendored js-yaml (./vendor/js-yaml.cjs) under { schema: FAILSAFE_SCHEMA,
json: true }. Everything js-yaml does not do is layered on top, in one
place, carrying the seven design-doc consequences:

1. Empty value: a null js-yaml value is coerced to {} (matching legacy's
   own empty-value contract) so reconstructFrontmatter — which omits
   null-valued keys — still round-trips a bare `key:` line instead of
   deleting it. Verified live: progress: with no value survives
   parse -> reconstruct -> re-parse.

2. Unparseable no longer collapses to a bare {}: a new FRONTMATTER_UNPARSEABLE
   Symbol (exported), keyed exactly like the existing #3257 FULL_LINE_COMMENTS
   channel, is carried on the {} returned for malformed/refused YAML. Invisible
   to Object.keys/entries/JSON.stringify/for-in, so the 70 call sites that
   never inspect it are unaffected; wiring the 8 hasFrontmatter sites to
   consult it is a separate change, not done here.

3. Non-scalar object-list items (the four spellings of `- test: a b` that
   js-yaml collapses into one tree shape) are rendered as a canonical
   `key: value[, key2: value2]` string per item, keeping the existing
   array-of-strings value SHAPE. A full corpus differential over all 1702
   tracked markdown files found 11 residual divergences from the legacy
   parser (enumerated in the PR/report), most of them the parser now being
   MORE correct (a dropped quoted top-level key, the block-scalar/phantom-key
   defect, a dropped Unicode key).

4. The #1882 truncation probe still runs the one real parser, but derives
   its key count from js-yaml's own thrown error and mark.line when the
   whole region doesn't parse cleanly (the dominant real truncation shape:
   fence opened, well-formed keys, no closing fence). Verified against both
   the clean-parse and the exception-fallback path.

5. The #3257 comment channel now attributes each pending column-0 comment
   against js-yaml's own parsed top-level key list (matched by literal key
   text, in document order) instead of the legacy ASCII-only key regex, so
   a comment above a Unicode key attaches correctly.

6. Anchors, aliases and merge keys are refused outright (a raw-text
   pre-scan, since FAILSAFE_SCHEMA still resolves them) — corpus occurrences
   today: zero. A 7-line billion-laughs fixture is verified refused rather
   than expanded.

7. A literal U+0000 is swapped for a private-use sentinel before the parse
   and restored in every resulting string afterward, since js-yaml rejects
   NUL unconditionally under every schema.

escapeDoubleQuoted is deleted and reimplemented via js-yaml's dump()
(forced double-quoted style), with control-char hex escapes lowercased to
keep serialized output byte-stable (#1779 emitted lowercase); it keeps its
exported name and signature for its two other call sites (commands.cts,
runtime-artifact-conversion.cts), which need no change.

frontmatterDeepEqual, the comment channel, sliceTopLevelFrontmatterSegments,
regenerateFrontmatterKey's guard, noOpObjectListSetError and
parseMustHavesBlock are all unchanged — retiring them is fork (b) and is
not this phase.

SECURITY.md/UI-SPEC.md/VALIDATION.md wrote frontmatter placeholders as
bare {N}/{phase-slug}/{date}, which is valid YAML flow-mapping syntax
under the vendored js-yaml parser, not the literal placeholder text
intended. Quote them so they parse as strings.

Wire the FRONTMATTER_UNPARSEABLE Symbol (exported but unused) at the
8 call sites in state.cts/state-transition.cts that compute
hasFrontmatter via Object.keys(extractFrontmatter(...)).length > 0 and
reassemble the document without a frontmatter block when false. That
check conflated 'no frontmatter' with 'unparseable frontmatter' (both
parse to {}), so a document with a merge-conflict marker or refused
alias in its frontmatter had that block silently dropped on write.
Each site now preserves the exact raw bytes stripFrontmatter removed
when the marker is set, leaving the genuinely-empty case unchanged.

frontmatter block independently, so the marker binds at the transform layer.

The maintainer chose the full semantic migration over splitting the rule into
its own epic or patching the scanner, so section 8.1 is answered as "the fork

guard surface with the counted result: excluding vendored third-party lines the
hand-maintained surface is net +307, and frontmatter.cts grew by 68 lines
despite four functions being deleted, because the compatibility layer over
js-yaml is larger than the scanner it replaced. Section 8.1's stated benefit is
therefore not delivered as written; what improved is the kind of code
maintained, not the amount. Decision 6 requires recording that rather than

ADR-3473 §8.1 review findings, addressed inline:

Finding 1 (BLOCKER): refuseAnchorsAndAliases was a raw-line regex that matched
only the bare-key spelling (key: &x). A quoted key ("a": &x), a flow mapping
({b: &x}) and a flow sequence ([&x, *x]) all define/use the SAME anchor
mechanics while never matching that line shape, so the exact expansion the
guard exists to stop went straight through unrefused (a 303-byte quoted-key
bomb expanded to ~35.8MB). Replaced with js-yaml's own `load` `listener`
callback, which reports `state.anchor` for every event belonging to an
anchored node in every spelling, and throws from inside the callback to abort
before any expansion (~1-2ms vs full expand-then-discard). A merge key with
an alias is still refused (merge always requires a previously anchored node,
so the alias itself trips the listener); a bare merge key with NO alias is no
longer separately refused, documented as intentional: FAILSAFE_SCHEMA never
resolves `!!merge`, so it carries no expansion risk. Table-driven tests added
for all four bypass spellings + merge key, plus a quoted-key-spelled
billion-laughs fixture registered in the adversarial matrix and README.

Finding 2: src/vendor/js-yaml.d.cts's docblock falsely claimed anchors/
aliases were "simply UNREACHABLE from typed code" through the twin. Corrected
to state the truth: anchor/alias resolution is document-level `load`
mechanics reachable through exactly the declared surface, and refusal is
enforced at RUNTIME (Finding 1's listener), not by the type surface.

Finding 3 (MAJOR): the null-byte sentinel (U+E000) round-trip was
non-injective — restoreNullBytesDeep rewrote every U+E000 in the parsed tree
back to NUL, including one the document author legitimately wrote, silently
corrupting it. Now refuses outright whenever the raw region already contains
U+E000 (consistent with the existing anchor/merge-key refusal path), making
the substitution provably injective. Tests added for a real NUL alone
(preserved), a pre-existing U+E000 alone (refused, not corrupted), and both
together (refused, not merged into one byte).

Finding 4 (MAJOR): scripts/lint-vendored-deps.cjs's `srcTwin` field was dead
for a hand-authored row (only read inside the upstream-verbatim branch) —
exactly how Finding 2's stale docblock drifted unnoticed. Added
checkHandAuthoredTwin: every value-level export the twin DECLARES must be an
actual own property of the vendored runtime module at require-time. Tests
added, including a sensor that a declared-but-nonexistent export IS caught.

Finding 5: the existingFm/hasFrontmatter/stripFrontmatter/fmPrefix/
unparseableFm/reassemble preamble, copy-pasted at 7 sites in
state-transition.cts plus a sixth hand-inlined copy in state.cts's
cmdStateCompletePhase, is now one exported helper
(beginFrontmatterReassembly) every site routes through, including the
hand-inlined one. Three call sites (beginPhaseCore, patchCore, updateCore)
keep a literal `body = stripFrontmatter(content)` assignment alongside the
helper call so scripts/lint-state-write-path-drift.cjs's single-hop backward
scan (which does not chase aliases) still sees the strip; stripFrontmatter is
pure/idempotent so the extra call changes nothing observable.

Finding 6: corrected the frontmatter.cts docblock's stale "wiring is a
separate change" claim (the 8 call sites are wired on this branch) and the
changeset's backlink from (#3473) to (#3881).

Finding 7: fixed the lint:ci failures blocking the gate — an
@typescript-eslint/only-throw-error violation from throwing a bare Symbol as
the anchor-detected signal (now a real Error subclass), unused-var warnings
left over from the Finding 5 refactor, a lint-test-file-count cap exceeded by
two migration-specific test files (allowlisted with justification), and the
lint-state-write-path-drift false positive from Finding 5's helper (fixed
above). tests/frontmatter-golden-parity.test.cjs:117's execFileSync already
carried an explicit timeout; no change was needed there.

Golden fixture: added a golden entry for the new
anchor-alias-bomb-quoted.md fixture ({} — matches what the legacy line
scanner would also produce, since it independently dropped every quoted
top-level key). No other corpus document diverges: real .planning/ documents
carry zero anchors/aliases/merge keys/U+E000 today.

ASCII-only key regex for the Unicode fixture; updated to require the 相
key's value now that js-yaml has no such restriction. Audited the rest of
the file for other pre-migration pins (block scalars, quoted keys,
flattened values, empty values, duplicate keys, unclosed blocks, null
bytes) by execution against real fixtures; found none regressed.

Finding 2: parseYamlRegion and escapeDoubleQuoted renamed to
parseGuardedYamlRegion and escapeDoubleQuotedScalar in src/frontmatter.cts
so no function still answers to the deleted hand-rolled scanner's name
(ADR-3473 §8.1 "deleted, not patched"). escapeDoubleQuotedScalar's three
external call sites (src/commands.cts, src/runtime-artifact-conversion.cts)
updated in the same change — a mechanical rename, not an ADR-amendment
matter.

Finding 3 (BLOCKER): fixed a real crash and a silent data-loss bug found
by execution. A top-level key named constructor/__proto__/toString/
valueOf/hasOwnProperty crashed reconstructFrontmatter (bracket read
resolving an inherited Object.prototype member); a key literally named
__proto__ was silently DROPPED entirely (bracket assignment on an
ordinary {} invoked the inherited __proto__ setter instead of creating a
data property). Fixed by building every parsed Frontmatter object with
Object.create(null), and replacing an `in` check with hasOwnProperty.call
in propagateCommentChannel. Added round-trip tests for all five hostile
keys, each with its own leading comment.

Finding 4 (MAJOR): escapeDoubleQuotedScalar's docstring falsely claimed
full byte-stability across the migration. Verified by execution: BEL/NUL/
NEL/NBSP/LS/PS/BOM now emit YAML-named escapes instead of the old hex/raw-
literal forms. Proved round-trip equivalence (each escape re-parses to the
exact source codepoint) and corrected the docstring. Found and fixed a
related real defect while verifying: a lone UTF-16 surrogate was emitted
BARE (scalarNeedsDoubleQuoting didn't trigger), producing genuinely
unparseable YAML that silently collapsed to {} on re-read — extended
scalarNeedsDoubleQuoting to route surrogates through the quoted+escaped
path.

Finding 5 (MAJOR): countKeysBeforeTruncation went silent on 4 real
truncation shapes (unquoted colon, open flow collection, mis-indented
sibling key, refused anchor). Root cause: the mark-based prefix recovery
excluded the very line whose key needed counting, and a mark-less refusal
never entered the recovery branch at all. Fixed by taking the max of two
lower bounds: the longest parser-verified line-prefix, and a raw-text
count of key-shaped lines (reusing the same key-shape pattern this file
already uses for isFrontmatterShaped). Extended test-matrix row A5
table-driven over all 4 regressed shapes.

Finding 6: the design doc's claim that no test owned the #3594 adversarial
fixture corpus was false — consolidation epic #1969 had already folded it
into tests/frontmatter.test.cjs. An earlier commit on this branch
re-created a standalone duplicate under that false premise; folded its
genuinely-new coverage (fixture-ownership check, anchor-bomb fixtures,
block-scalar B1/B2 rows) into frontmatter.test.cjs and deleted the
duplicate file. Corrected the false claims in 40-design.md §3.3.1 and the
ADR's §8.1 note, including the roadmap-sibling claim (no such file exists).

Finding 7: the golden serializer sorted object keys, making it structurally
blind to the key-order-parity invariant ADR-3473 §8.1 actually claims.
Made it order-preserving and regenerated the golden fixture from a
standalone compile of the legacy (pre-#3881) parser at ddde001af; the
current parser matches it with zero undocumented divergences, confirming
key-order parity genuinely holds. Extended row A2 table-driven across 6 of
the remaining 7 transitionCore kinds (all pass) plus documented, by
execution, a newly-discovered 8th-site regression: state.cts's
cmdStateCompletePhase calls the same preservation helper but its result is
clobbered by a later unconditional resync — filed as a distinct finding
rather than fixed here (touches syncAndPreserveStateMd, outside this
change's verified scope).

Characterization (executed, before/after shown): case (b), not (a). The
frontmatter FENCE survives — `state complete-phase` on a conflict-marked
STATE.md returns success and a well-formed, freshly-derived frontmatter
block, not a document with no frontmatter at all. But the block's actual
content (the merge-conflict markers, and with them any signal to a human
that the document was in conflict) is silently discarded and replaced.

Root cause was two clobber sites, not one:

1. syncStateFrontmatter (src/state.cts) re-parses the already-preserved
   `transformedContent` from readModifyWriteStateMd, finds {} + the
   FRONTMATTER_UNPARSEABLE marker, and unconditionally rebuilt a fresh
   frontmatter block from the body anyway.
2. Even after (1) is fixed, applyPostSyncPreservation's own
   postFm/applyStatePreservation/authoritativeFm-reassertion machinery
   re-extracts frontmatter from syncedContent, restores curated fields
   from the pre-write snapshot, and reconstructs a NEW block again —
   confirmed live via `state begin-phase`, which still lost the markers
   after fixing (1) alone.

Both are now guarded by the same predicate (isUnparseableFrontmatter,
checking FRONTMATTER_UNPARSEABLE): when the ORIGINAL frontmatter did not
parse and the caller is not on ADR-3408 §8.3's closed "body wins" list,
both functions return their input content unchanged rather than
re-deriving over it. The closed list (cmdStateSync #905,
/gsd-health --repair's REGENERATE_STATE, both routed only through
writeStateMd, which never reaches applyPostSyncPreservation and passes
sanctionedPermanentEmptyFallback=true to syncStateFrontmatter) is
untouched — neither widened nor narrowed; verified by execution that
`state sync` still overwrites the conflict-marked block exactly as before.

Other verbs sharing the same readModifyWriteStateMd path were checked and
were equally affected before this fix: state update, query state.patch,
and state begin-phase all lost the conflict markers (RED, shown by
execution), and all three now preserve them (GREEN). Covered table-driven
in tests/feat-3881-yaml-parser-consequences.test.cjs's new A2b describe
block, which drives the real CLI verbs via runGsdTools — not just the pure
transitionCore layer the earlier A2 rows exercised — plus a control
asserting state sync's body-wins contract is unchanged.

Root cause of the bulk of the 88 remote-runner failures: extractFrontmatter/parseGuardedYamlRegion handed back Object.create(null) trees for prototype-pollution safety, but assert.deepStrictEqual compares prototypes, so every assertion against a plain object literal failed (57 frontmatter.unit.test.cjs + 5 frontmatter.test.cjs + others). Fixed by keeping the internal construction null-prototype (unchanged) and converting to a plain-prototype tree via Object.defineProperty (never bracket assignment, so __proto__/constructor/toString keys stay safe) at the parseGuardedYamlRegion/unparseableResult return boundary only; the internal FULL_LINE_COMMENTS Symbol channel is copied by reference, not recursed, so its own __proto__-safety is untouched.

Per-class fixes: (1) bomAcrossArtifactTypes was the same prototype bug, no separate code change needed. (2) frontmatter-cli #1660: added objectListFieldWouldLoseData, a broader lossy-field detector alongside the existing byte-identical noOpObjectListSetError -- js-yaml's flattenObjectListItem now correctly includes every sub-key of an object-list item (a real bug fix over the legacy scanner, which silently dropped every field but the first), so a set that drops that now-included data is no longer byte-identical to the original and needs its own guard. (3) uat.test.cjs: updated the pinned expectation for the human_verification quote-stripping artifact -- js-yaml resolves quoting correctly where the legacy regex left an unbalanced quote; documented as an intentional, non-lossy behavior change. (4) smart-entry: added a fallback-only loadWithAmbiguousColonRepair so a column-0 key: value line whose value itself contains an unquoted colon (the #2571 hand-edited-STATE.md shape) round-trips instead of failing the whole frontmatter block closed. (5) frontmatter.unit.test.cjs bracket-array leniency: added a second fallback, repairMalformedInlineArrays, restoring the legacy scanner's tolerant inline-array handling (consecutive/blank commas, unclosed bracket) -- both repairs run ONLY after the primary parse already threw, so well-formed documents are unaffected. (6) prompt-injection-scan: src/frontmatter.cts had a literal U+FEFF BOM embedded in a comment illustrating the #2977 fix; replaced with the U+FEFF text escape. (7) eslint-glob-coverage: allowlisted the new src/vendor/js-yaml.d.cts vendored type declaration, same precedent as the existing re2js.d.cts entry. (8) frontmatter-golden-parity: git ls-files *.md now runs with -c safe.directory=* (process-scoped) so it survives the remote runner's dubious-ownership check without a persistent git config write.

This repo merges ~21 commits/day; a 14-day sample measured 937 touches of the
exact files (commands/gsd/*.md, gsd-core/workflows/*.md, agents/*.md,
docs/*.md) the prior golden pinned by tracked path. Any PR editing one of
those files' frontmatter for reasons unrelated to the parser (an
argument-hint addition, an allowed-tools tweak) turned the suite red, and the
reflex fix -- "regenerate the golden" -- overwrote the very snapshot meant to
catch a real regression. Excluding .changeset/** was not enough; the design
itself was wrong: a regression fixture must not be keyed to mutable repo
paths, and a single 376-entry JSON every such PR touches is also a
guaranteed merge-conflict surface.

Rebuilt the fixture to carry its own documents: each of 51 entries stores a
stable id, literal documentText (shrunk from a real ddde001af-era corpus
document), and an expectedParse captured independently from the
pre-migration legacy parser (git show ddde001af:src/frontmatter.cts,
compiled standalone against its byte-identical sibling modules). The test
reads no tracked path, shells out to no git command, and enumerates no tree
-- a PR editing commands/gsd/help.md cannot affect it. Every entry's
reconstruction was verified at capture time to reproduce both the current
and legacy parser's output on the original document; 0 of 51 candidates
were dropped by that check (1, the deliberately-unterminated
unclosed-block.md adversarial fixture, has no closing fence to truncate at
and is stored unshrunk). Kept the 5 documented DIVERGENCES rows (now
diverges:true entries) and the D2 order-preserving structural serializer
that keeps the comparison from passing vacuously; dropped the
tree-enumeration helpers, the coverage floor, the post-capture-skip logic,
and the vanished-file check -- all artifacts of the path-keyed design.

Five rows in tests/lint-vendored-deps-manifest.test.cjs failed on
windows-latest CI: the test passed absolute scratch-file paths into
compareFiles()/checkRow(), whose helpers joined every input onto ROOT
via path.join(ROOT, rel), producing garbage when the input was already
absolute. It surfaced on windows-latest specifically because GitHub's
Windows runners checkout the repo on a different drive than TEMP, so
path.relative(REPO_ROOT, tmpFile) returned the absolute path unchanged
(no relative traversal is representable across drives) rather than the
relative form the test assumed. The remote gsd-test runner this repo
gates pushes on is Linux-only and could never have caught this;
GitHub CI's windows-latest job is the only signal that does, and it did.

Fixed the helper itself (scripts/lint-vendored-deps.cjs's new
resolvePath()) to treat an already-absolute input as absolute-in,
absolute-out instead of silently mis-joining it, and updated the test
to pass the scratch file's absolute path directly rather than relying
on a relative conversion that is not always representable. Kept every
mutation-sensor assertion intact and added coverage proving
resolvePath is a no-op for relative inputs and correctly passes
absolute ones through unchanged.

Reverts 9638f2936 and its changeset. The change was wrong and the revert is
the correction.

This repo distinguishes two error mechanisms deliberately. error() in
src/io.cts writes to stderr and calls process.exit(1) -- the hard-failure
path. output({error: ...}) writes a JSON result envelope to stdout and returns
normally with exit 0. The reverted commit converted 23 result-envelope sites
into hard failures, which is a different contract, not a bug fix.

tests/state-contract.test.cjs's errorPathDoesNotPublish asserts the envelope
contract directly -- a failing command exits 0 with a JSON error envelope and
must not publish state.json -- and the remote matrix run caught it along with
four cases in the QA scenario walk. Thirteen tests in tests/state.test.cjs that
the original commit rewrote were encoding that real contract, not the bug it
claimed; they are restored.

Whether an error envelope on stdout with exit 0 is the right CLI design is a
genuine question, and it is section 8.4's rule ('failure is a value') with its
own phase. It is not something to flip inside this PR.

The Stryker (frontmatter) shard hit the documented 15-minute (900s) shard
cap. Root cause is NOT row-level spawn overhead (contrast the #2790/
core-utils precedent): the three shard test files' own logic runs in
~413ms total (356+30+27ms) with all 392 assertions passing. Instead,
src/frontmatter.cts grew from ~825 to 1496 lines (+671/-187) migrating to
the vendored YAML parser, proportionally growing the mutant count Stryker
generates for gsd-core/bin/lib/frontmatter.cjs. Stryker's command runner
bills the full 'node --test <3 files>' invocation once per mutant, and
node:test's default per-file process isolation forks a child process for
each of the three files on every one of those invocations — pure fork
overhead multiplied by a much larger mutant population.

Fix: scripts/mutation-matrix.cjs COVERED.frontmatter now declares
isolation: 'none', and .github/workflows/mutation.yml passes
--test-isolation=${{ matrix.isolation }} (defaulting to 'process' — i.e.
unchanged behavior — for the other 8 shards, which were not individually
audited for cross-file state leakage under shared-process execution).
Measured locally via node:test's run() API on the exact 3-file set:
isolation:'process' took ~593ms vs isolation:'none' ~478ms for the same
392 passing assertions. The true CI-shard number can only be confirmed
on the GitHub Actions run (Stryker cannot run locally, and 'node --test'
is hard-blocked in this environment).

timeoutMinutes field threaded through mutation.yml's job-level
timeout-minutes the same way isolation is threaded, and removes the
prior isolation:'none' override (re-measured at this file-set size, its
savings are within run-to-run noise, not worth the unaudited
cross-file-state-leakage risk).

7a02f98574 fix(#3493): confine key_links from:/to: to the project directory (#3506)
`cmdVerifyKeyLinks` resolved `from:` and `to:` with `path.join(cwd, <value>)` where the value comes verbatim from plan YAML. `path.join` normalizes `../` rather than rejecting it, so a plan travelling with a repository could name any file the process can read, and the command reports whether the link's `pattern` matched it — an arbitrary-file-read oracle reachable from `verify-phase`.

Both reads now go through `validatePath` in src/security.cts, the existing realpath-based confinement seam already used at 11 call sites. Not a missing capability — a bypassed one.

Two defects in that seam, found by adversarial review and fixed here because they affect all 11 callers:

1. A dangling in-project symlink escaped confinement. A link to an EXISTING outside path was refused (realpath lands outside) while a link to a MISSING outside path took the parent-resolution fallback and was accepted — an existence oracle for arbitrary absolute paths. lstat succeeds on a dangling link and throws ENOENT on a truly absent path; an unresolvable link is now refused. A symlink resolving inside the project is still accepted.

2. A canonicalized base was compared against an uncanonicalized path when a file and its parent were both missing, wrongly refusing legitimate in-project paths on any non-canonical cwd (every macOS temp dir). This was a live regression in this PR: the wave-pending classification (#1202) depends on the not-yet-created case. Resolution now walks up to the nearest existing ancestor.

Two adjacent aborts fixed: the from: read sat outside the per-link try, so a non-ENOENT errno killed the whole command; and an empty from: read the cwd directory, throwing EISDIR. Both now fail per-link.

The issue was filed as a fourth ADR-0174 consolidation loss. It is not one — validatePath/requireSafePath never went away, only the SDK's name for the concept did. This is an instance of epic #3473's F2 family. The ADR-0174 loss count is three.

895d9df96d fix(#3477): run untrusted key_links patterns on a linear-time engine (#3496)
`cmdVerifyKeyLinks` compiled `must_haves.key_links[].pattern` from plan frontmatter with `new RegExp()` and tested it against whole file contents, so a nested-quantifier pattern such as `(a+)+$` hung `verify-phase` indefinitely (CWE-1333). JavaScript has no regex-execution timeout.

Untrusted patterns now run on RE2 (re2js), whose match time is linear in input length — the class is closed by the engine, not by a heuristic screen. The screen lost in the ADR-0174 consolidation was deliberately NOT restored: it never worked, since `(a|a)*$`, `((a+))+$`, `(a+){2,}$` and `(a{1,3})+$` all evade it. A refused pattern's matcher returns false for every input, so it cannot report a match no matter what the caller does.

The engine is vendored at gsd-core/bin/lib/vendor/re2js.cjs because gsd-core/bin/** is copied into installed trees with no node_modules; runtime dependencies are unchanged. New ESLint rule local/no-external-require-in-bin enforces that invariant, which had been documented in a comment since the #3024/#2071 bug class and enforced nowhere.

Backreferences and look-around are unsupported by RE2 by construction — disclosed in a Changed changeset.

* feat(#3415): ship local/no-unbounded-quantifier, burn down ReDoS class

Phase 4 of epic #3212 (ADR-3212 §5/§7, the final phase). New rule flags
an unbounded */+/{n,} quantifier over a broad character class
([\s\S], dotAll ., or a 1-2-unit negated class like [^\n]/[^)\n] — the
exact #2128-fixed shape) applied to a regex whose match target is
data-flow-traced to readFileSync content.

eslint-rules/lib/readfilesync-trace.cjs extracts the data-flow tracer
shared with no-crlf-fragile-split (Phase 2) rather than a second copy
— no-crlf-fragile-split refactored onto it with zero behavior change,
parity-tested.

Real triage, not 798 mechanical edits: the ADR's census (2026-08-08)
screened every unbounded quantifier in the tree unscoped. Correctly
scoped to readFileSync-derived content (matching Phase 2's own G2/G3
scoping), the rule found 162 real hits across two detection waves — the
second wave (93) surfaced only after a genuine off-by-one bug in this
rule's own first draft was caught while writing its RuleTester tests
and fixed (the bug silently missed every directly-quantified [\s\S]*
with no gap before the quantifier — exactly the class this rule exists
to catch). 3 hits landed in production src/ (commands.cts, milestone.cts,
roadmap.cts) and were each empirically timed against adversarial input
(matching #2128's own measured-not-assumed precedent) — all confirmed
linear-time/benign, left unbounded with a measured-evidence comment
rather than mechanically bounded. The remaining 159 are test-file
fixture parsing (test-author-controlled, fixed-size content, not
adversarial input) — each suppressed with a specific, non-generic
reason. Zero functional behavior changed anywhere in this diff.

tests/no-pending-3212-markers.test.cjs locks the epic's own closing
invariant (ADR §7: "assert zero pending #3212 markers remain") — ground
truth confirmed trivially true today (no phase left any such marker
behind), now regression-locked going forward.

had one of its own: hasUnboundedBroadQuantifier's negated-class inner
scan walked from each `[^` occurrence to the next `]` (or EOF) with no
bound, while the outer loop only ever advanced by one character — O(n²)
total work on a pattern with many unclosed `[^` runs. Runs unconditionally
inside checkPattern on any `new RegExp('literal string')` argument in any
linted file, before the (cheap) readFileSync data-flow gate — so a single
crafted string literal, no valid regex syntax required, could make
`npm run lint` / CI hang.

Empirically confirmed both the bug and the fix: pre-fix, n=4000/8000/
16000/32000 chars took 30.8/115.6/463.8/1874.3ms (~4x work per 2x n,
quadratic); extrapolated, the 300000-char repro from the finding would
run ~165s. Post-fix (bail the inner scan once units exceeds the rule's
own 1-2-unit scope, rather than continuing to hunt for a closing `]`),
the same 300000-char input runs in 8.7ms via the real rule module,
independently reconfirmed at 18ms via a fresh Linter.verify() call.

New regression row in tests/no-unbounded-quantifier.rule.test.cjs
asserts the RuleTester run on a 50000-char adversarial pattern
completes and returns a defined result — no wall-clock assertion
(CLAUDE.md Clock Seams / local/no-elapsed-assertion).

d1760e3c31 refactor(#3309): migrate cmdValidateHealth onto the rule table
Replaces cmdValidateHealth's hand-rolled addIssue/switch accumulation
(961 lines) with buildPlanningSnapshot -> evaluateRules -> map to the
legacy {code, message, fix, repairable} shape, bucketed by severity.
Two pre-checks (home-dir E010/I010, .planning/-root-missing E001) stay
outside the rule table entirely, per ADR-3180 §8.2 rule 4 ("no
precedence system") — building "some rules suppress others" into the
table would itself be the forbidden precedence system.

W024 (STATE.md commit-age freshness) also stays outside the table:
its committed rule is a documented permanent no-op (readStateHeadFreshness's
git-log shell-out is ambient I/O a Rule.check may never perform, and no
PlanningSnapshot field carries a commits-behind count). Migrating onto
the rule table as designed would have silently regressed 7 passing
tests in tests/health-validation.test.cjs — found while wiring this
function, kept as a real check in the wrapper instead (same I/O
license applyRepairs already relies on), fixed inline per this repo's
no-defer policy rather than accepted as a silent loss.

Ports the real repair-handler bodies (createConfig/resetConfig,
regenerateState, addNyquistKey/addAiIntegrationPhaseKey,
backfillMilestones) into health-diagnostic.cts's applyRepairs,
replacing the skeleton's stub. DESTRUCTIVE-risk remedies
(resetConfig/regenerateState) are refused by --repair — a disclosed
breaking change; repairable now means "an automatic repair will
actually run," not merely "a remedy exists to describe," so E004/E005
now report repairable:false. --backfill alone now actually triggers
backfillMilestones, fixing a latent bug where its gate was unreachable
without --repair also being set (verify.cts:2504, confirmed dead code
pre-migration).

Test updates distinguish the two explicitly-authorized behavior
changes (DESTRUCTIVE refusal, backfill-alone fix, W021->W026 split)
from preservation — every changed assertion is commented with why, and
new regression tests were added for both changes plus W021/W026
mutual independence. Drift-guard bookkeeping (bypass-baseline shrunk
to the one disclosed W024 exception, milestone-window and
phase-enumeration exemptions, test-file-count allowlist) updated for
the relocated/new functions this migration introduces.

* test(#3338): fix orthogonal-review findings — Wave 6 fold

Standards-axis review found a real structural defect in two files, both
the same root cause and both fixed here:

- tests/update-workflow.test.cjs: the folded:issue-815-update-next-channel
  wrapper's closing brace was placed after the file's pre-existing tail
  instead of before it, making the already-established folded:bug-2470
  and folded:bug-3130 wrappers CHILDREN of issue-815's block in the test
  hierarchy instead of independent siblings — confirmed via an actual
  node --test run showing the mislabeled TAP nesting. Moved the closing
  brace to the correct position; all three fold wrappers are now
  top-level siblings again (verified via node --test, TAP hierarchy
  correct, 10/10 tests, 5 suites, identical count before and after).
- tests/plan-phase-drift-guard.test.cjs: same mistake in the other
  direction — folded:issue-2762-plan-reviews-chunked was spliced inside
  the pre-existing folded:bug-2492-context-coverage-gate wrapper instead
  of after it. Fixed the same way (227/227 tests, 38 suites, identical
  count before and after).
- Reverted unnecessary 815-suffixed local renames (assert815/fs815/etc.)
  introduced by the fold — the wrapper is genuinely block-scoped once
  correctly closed, so no collision existed (same class as Wave 4's
  __foldSetNested finding).

No test() count changed in either file. No production code touched.

convention) produced a spurious W007 and a spurious 'Gap in phase numbering:
N → 999'. Add failing-first regressions for both surfaces + the gap check, each
with a non-sentinel orphan negative-space guard.

* fix(#3225): guard W006/W007 + consistency + gap loops with isSentinelPhaseId

cmdValidateHealth's W006/W007 loops, cmdValidateConsistency's parallel disk↔
roadmap loops, AND its gap-numbering check never got the isSentinelPhaseId guard
that phase.cts has at 10+ sites (#2786/#2949). So any repo using the sentinel-id
convention (999.x backlog/interim, 0.x drafts) got a permanent spurious W007 and

(violates the convention) or delete (destroys archived work).

Add isSentinelPhaseId to the phaseIdMod destructure and skip sentinel ids in:
W006 + W007 (cmdValidateHealth); the two plain-warning disk↔roadmap loops and
the gap-numbering integerPhases filter (cmdValidateConsistency — same bug family,
folded in inline per no-silent-defer). Additive only: non-sentinel orphans and
real numbering gaps still warn. The gap-numbering guard was surfaced by the
isolated review (a 999-interim dir would otherwise create a false 'N → 999' gap).
Same family as #3167 (since fixed).

* test(#3145): bound the installer/runtime cluster onto the process seam

Migrates 156 unbounded sync spawn sites across 47 files. Allowlist 120 to 73.

Timeouts are sized from evidence already in the tree rather than a house
default, because this wave spawns installers rather than git plumbing and an
undersized bound does not catch a hang -- it manufactures CI flake, which is
worse, since a flake gets re-run instead of investigated. install.test.cjs
records a real spawnSync ETIMEDOUT at a 60000ms cap on a loaded bench while
another lane passed the same commit in 12.7s, so full installs are bound at
120000ms against that recorded incident.

Also adds an auditable escape to the guard's timeout ceiling. The 600000ms
cap was set in #3143 from partial evidence, but fragment-single-edit-
propagation carries a documented, load-tested 900000ms bound on a run that
chains a full build plus eight generators -- the guard would have rejected a
correct timeout the moment that file left the allowlist. A value above the
ceiling is now permitted only with an inline allow-spawn-timeout-ceiling
marker carrying a non-empty reason. It raises the ceiling; it never waives
the requirement for a bound, which is asserted directly.

install-shared.cjs keeps its hand-rolled assert rather than routing through
throwIfFailed: its message embeds both streams, and throwIfFailed carries
only a trimmed stderr. The message now also names the outcome, so a bounded
timeout reads as such across its 38 importers instead of as
expected null to equal 0.

* test(#3145): extract class-norm timeouts and correct the build-hooks sizing

A pre-PR review found 52 copies of four class-norm timeout constants across
this wave. These are not per-suite fixture bindings -- they are shared facts
about how long a class of subprocess takes, derived from a recorded bench
incident. That norm already moved once (60000 to 120000 after a real
ETIMEDOUT), and 52 copies would have drifted the next time it moved.

Extracts tests/helpers/timeouts.cjs, where each norm is justified once, and
converts the copies. A site that genuinely differs -- a real tsc compile, or
regen:derived -- keeps its own local constant with its own justification.

Also corrects a misclassification: scripts/build-hooks.js was sized as a
build at 120000 in twelve places and 60000 in another, but it compiles and
bundles nothing. Its own header says no bundling needed; it copies pre-built
files and syntax-checks them with vm. Three different values bounded one
script; now there is one.

* test(#3145): fix red CI — lint self-match and a Windows chunk overrun

Two failures on PR 3176.

lint-allow-test-rule-refs read a RuleTester fixture as a real exemption. The
fixture exists to prove an unrelated marker does NOT suppress the rule, so it
carries that marker's literal text as test data. Split via concatenation, the
same idiom no-unbounded-spawn-allowlist.test.cjs already uses for its own
self-match problem. The explanatory comment needed the same treatment.

The Windows shard 3/3 chunk was killed at its 600000ms budget. Output stopped
seven minutes before the kill, so this was an overrun rather than a slow
chunk: regenDerivedPropagatesSingleFragmentEditWithNoSecondSourceSurface runs
regen:derived bounded at 900000ms, which is larger than the whole chunk
budget, so the chunk killer always fires first and it can never complete
there. Both the test and that bound predate this change; modifying the file
pulled it into the Windows targeted set and exposed it. Skipped on Windows
with the reason recorded; the Linux lanes cover it. The 900000 bound and its
ceiling marker are unchanged -- they are correct.

* test(#3145): refresh the stale test-timings cost table

The Windows shard was killed at its 600000ms per-chunk budget. run-tests.cjs
packs chunks by measured duration from tests/test-timings.json, and an
unknown file falls back to the table's median weight -- advisory by design,
but it silently underweights exactly the files that matter.

Four of the failing chunk's 22 files were absent from the table, including
the two heaviest: fragment-single-edit-propagation.install.test.cjs at 230s
(it runs regen:derived) and agent-fragments-emission.install.test.cjs at 79s.
Both were weighted as average, so the chunk's total weight read 53.68 against
a budget of 60 and the packer produced a single chunk.

Regenerated from a passing full-suite run, per the remedy the script itself
documents. 700 to 770 entries, 70 added, 0 dropped -- verified, since
gen-test-timings.cjs replaces the table wholesale rather than merging.

Proven against the real packer: the same 22 files now weigh 103.91 and split
into two chunks. No logic, budget, or timeout was changed; raising a budget
to make a red gate pass is not a fix.

Migrates 180 unbounded sync spawn sites across 19 files. Every previously
unbounded call now carries an explicit timeout with a comment giving the
number and why.

The migration is not a callee swap. execSync and execFileSync throw on a
non-zero exit and the seam never does, so each site was classified first:
sites that rely on the throw route to gitOrThrow, and sites that already read
.status to detect an EXPECTED non-zero -- an intended cherry-pick conflict, a
rev-parse outside a repo driving a skip -- route to the never-throwing runGit
instead, which would otherwise throw on exactly the exit being probed for.

Two same-named git() helpers in worktree-cleanup.test.cjs have different
return contracts, one trimmed and one raw; both are preserved rather than
unified.

Collapses five hand-rolled throw wrappers onto one throwIfFailed in
git-fixture.cjs, which gitOrThrow now also uses so the shape cannot drift.

Allowlist drops 139 to 120; BASELINE lowered to match.

* test(#3144): fix pre-PR review findings

Documents throwIfFailed in the CONTEXT.md glossary and CONTRIBUTING.md --
it became the shared throw mechanism without either doc naming it.

Routes the sixth and seventh hand-rolled copies of the throw shape through
throwIfFailed (worktree-baseref-install, worktree-safety-reap); the first
consolidation missed both.

Converts ci-rebase-check's 8 fixture-setup calls from unchecked runGit to
gitOrThrow so a failed setup step aborts where it fails rather than
surfacing later as a confusing failure against the wrong subject.

Adds 12 direct unit tests for throwIfFailed, which until now was only
exercised transitively.

Splits verify.test.cjs's non-git grep/sed bound off GIT_TIMEOUT_MS.

7dd9e59f6b test(#3090): stop exempting violations under categories that do not fit
An allow-test-rule annotation citing a category that does not apply is worse
than no annotation, because it reads as reviewed. Eight were confirmed by
reading the assertions each one covered, and auditing the rest found five more
plus one refutation — a converter test whose wording described the wrong
mechanism while the covered assertion genuinely was deployed-text.

The instructive one used the CANONICAL string for the same mistake: STATE.md
command output labelled as a deployed artifact. A canonical string is not
evidence the category fits, which is why normalising strings alone would have
laundered the problem rather than fixed it. Every mapping the audit had inferred
rather than code-verified was spot-checked before rewriting, and the ones that
turned out not to fit were re-annotated rather than relabelled.

Fourteen STATE.md assertions had a typed extractor available all along and now
use it; their annotations came out because nothing needs exempting. Eight
assertions genuinely need a production change first — CLI stdout and stderr with
no structured mode — and are tagged pending-migration-to-typed-ir citing #3090,
which is what that category is for. It had zero real uses before this, while one
file carried a real citation to migration issue #2974 under a non-canonical tag.

Six annotations covered assertions that do no text matching at all. An exemption
for a violation that does not exist is noise that makes the real ones harder to
audit; those are removed.

atomic-write-coverage gains the annotation it always warranted — its own
docstring describes a structural-regression-guard while the file carried none.

Fifty-nine non-canonical strings across roughly thirty files are normalised, and
the allow-test-rule allowlist is regenerated to match. 472 annotations became
463: every one now uses a canonical category, and the two remaining
non-canonical strings are ESLint RuleTester fixtures, not annotations.

verify-summary's Pattern 1 matched any backticked path-like token with no
context check, so a prose mention of a future deliverable (`shared/types.ts`
in a 'next phase will add…' sentence) was checked for existence and its absence
failed the verdict on a healthy phase. #2685 added shape filtering but no
context check.

- src/verify.cts: both extraction patterns now require a claim label on the line
  (Created/Modified/Added/Updated/Edited/key-files). A bare prose mention no
  longer matches; genuine labeled claims still do.
- gsd-core/bin/gsd-tools.cjs: remove 'verify-summary' from SKIP_ROOT_RESOLUTION
  so relative claim paths resolve against the project root, not the raw cwd

* test(#1883): failing-first regression for findContextMdIn / listMilestoneArchiveDirs swallowing EACCES

Adds failing-first regression tests proving an unreadable dir is currently
swallowed as empty/null instead of surfacing the permission error. Covers
EACCES, EIO, the unchanged ENOENT empty path, the array fast-path, and both
CONTEXT.md forms. listMilestoneArchiveDirs is exercised in-process via a new
_listMilestoneArchiveDirs test seam (the validate command runs in a subprocess,
so an fs monkeypatch in the test process cannot reach it).

* fix(#1883): distinguish a permission/IO error from genuine emptiness in dir scans

findContextMdIn (src/planning-workspace.cts) and listMilestoneArchiveDirs
(src/verify.cts) catch-alled every readdirSync error into the empty marker
(null / []), conflating a genuine ENOENT ('nothing there') with an EACCES/EIO
failure ('can't read this'). An unreadable phase dir was silently reported as
'no CONTEXT.md' (discuss/plan gates wrongly skipped context) and an unreadable
milestones/ dir as 'no archives' (active-milestone resolution / archived-phase
filtering misbehaved).

Narrow each catch to ENOENT only — keep the long-standing null/[] contract for
genuine absence (Hyrum: empty path unchanged) and re-throw every other error so
it propagates to the caller's existing try/catch. All six findContextMdIn
callers either pass a pre-read string[] (no readdir) or sit inside a try block
that already handles readdir failures; the two listMilestoneArchiveDirs callers
live in the validate command path where errors reach the command error handler.

Exposes a _listMilestoneArchiveDirs test seam so the permission-error path can
be unit-tested in-process (the validate command runs in a subprocess, so an fs
monkeypatch in the test process cannot reach the private helper).

* fix(tests): delete stale emitted-drift ack for gsd-phase-researcher.md

Pre-existing base-branch defect, not part of #1883: commit 6932fb16d
(enhance(#1699): require read-and-cite provenance, #2768) landed the
gsd-phase-researcher.md growth onto next WITHOUT removing its now-obsolete
emitted-drift-ack.json entry. An ack explains a ripple in a PR diff; once the
change merges to next the ripple becomes part of the baseline, so the ack is
permanently stale for every subsequent PR — the emitted-attribution gate fails
on every branch off next with '1 stale acknowledgment(s) — the ripple they
explained is gone'.

The gate's own error message prescribes the fix: delete the stale entry, and
since gsd-phase-researcher.md was the only entry, delete the file (an empty ack
file signals nothing; its presence is the alarm). This unblocks the red base for
all in-flight PRs, not just this one. Folded inline per the bug-fixer directive
(one-line test-prescribed fix, does not bury the #1883 fix).

* test(#1883): use t.mock.method + t.after per CONTRIBUTING test conventions

Address code-review finding: replace inline try/finally in test bodies and

The artifact<->git check has existed since the beginning but was only ever
pointed at .planning/research/SUMMARY.md (new-project.md:1145,
new-milestone.md:425). Phase summaries -- the ones that actually claim
"I created these files" -- were never checked.

- extract verifySummaryCore from cmdVerifySummary: same checks, lifted out of
  the output() wrapper so callers consume {passed, checks, errors} directly
  instead of shelling out and re-parsing JSON; cmdVerifySummary is now a thin
  adapter over it
- validate.health gains advisory W025 per phase SUMMARY with missing files

Advisory only: appends to warnings[], never touches status escalation beyond
the channel's own warning semantics, the repair set, or readVerificationStatus.

Resolves both open questions from triage: (a) commits_exist is deliberately NOT
surfaced -- its hash pattern matches any hex-shaped token in prose, too loose to
show a user; (b) a phase carries N per-plan summaries plus a legacy bare
SUMMARY.md, so all of them are checked via the repo-wide filter.

Responds to the #2685 review. Three substantive changes.

Seam (Blocker 2). The check now runs in cmdPhaseComplete, the seam the
issue body cited (src/phase.cts:~1745), not validate.health. That channel
does exist: cmdPhaseComplete declares warnings[], populates it from the
UAT/VERIFICATION pre-scan, and emits it. The cycle objection raised
against the earlier deviation holds for state.cts only -- verify.cts has
no transitive import path to phase.cts, so phase.cts -> verify.cjs adds
no cycle (verified over every src/*.cts). Moving it also retires the
retroactive firing across all historical phases: this fires once, at
completion, for the phase being completed.

Extraction (Blocker 1). Pattern 2 now excludes [ and ] from its path
class. The SUMMARY templates prescribe a YAML flow sequence
(key-files.created: [a.ts, b.ts]) and the label matches case-insensitively,
so the class previously captured the literal [ and produced a candidate
that can never exist on disk -- firing on healthy projects built from the
templates GSD itself ships. Stripping frontmatter was the other offered
remedy; measured across all three shipped templates it is a no-op on top
of the exclusion, so it is not carried. Consequence named in-code: the
key-files block still is not read, which needs a real frontmatter parse.

Also narrowed to the noise classes confirmed in review -- globs, bare
hostnames, and paths resolving outside the project are skipped rather
than reported, and the containment guard the old comment claimed now
actually exists.

Budget (Majors 1 and 3). verifySummaryCore takes a checkCommits option;
phase completion passes false, so the discarded git cat-file probes are
not spawned at all. It also passes Infinity, so every referenced file is
reported instead of the first two -- a summary listing twelve files of
which nine are missing now says nine, not zero. The verb keeps its
historical 2-file default.

Tests (Major 2). The vacuous fixtures are gone with the health block.
The replacements use /-bearing paths that genuinely extract, and each
fix was mutation-checked: un-anchoring pattern 2, dropping the glob,
hostname or containment filter, forcing commit checking on, and
re-capping at 2 each fail at least one test.

* docs(#2572): describe the phase-completion SUMMARY artifact check

The W025 text under /gsd-health is withdrawn with the health seam; the
check is documented where it now runs, under `phase complete` in
docs/CLI-TOOLS.md.

Both the docs and the changeset previously overclaimed: they said a
referenced file not on disk is warned about, while at most two candidates
per SUMMARY were ever examined. The cap is gone at this seam, so the
claim now holds -- and the text states the limits that remain, rather
than leaving them to be discovered: the key-files frontmatter block is
not read, commit hashes are not resolved, and globs, URLs, bare
hostnames and out-of-project paths are skipped rather than reported.

46ba9ed464 fix(#2444): branch plan-structure validation on task type=checkpoint:* (#2473)
* test(#2444): failing-first regression for checkpoint:* plan-structure validation

Add acceptance-criteria tests covering the three canonical checkpoint task
types (human-verify, decision, human-action) plus an unknown-subtype
forward-compat case. Each canonical type must pass verify plan-structure
when it carries its type-specific required fields (per
gsd-core/references/checkpoints.md), and must be flagged when those
fields are missing. Non-checkpoint tasks keep the existing
<action>/<verify>/<done>/<files> requirements unchanged (AC3 regression
guards).

The existing 'errors when checkpoint task but autonomous is true' fixture
is updated to use the canonical checkpoint:human-verify triple
(<what-built>/<how-to-verify>/<resume-signal>) so it does not collide
with the new per-type validator; the assertion (autonomous is not false)
is unchanged.

* fix(#2444): branch plan-structure validation on task type=checkpoint:*

cmdVerifyPlanStructure unconditionally required <action>/<verify>/<done>/
<files> on every task, so every checkpoint:* task — which uses the
checkpoint convention's type-specific fields instead — was reported as a
structural error. Checkpoint-heavy phases produced walls of false findings.

The fix introduces two pure helpers in verify.cts:

  - extractPlanTaskInfos(content): single ReDoS-safe pass over
    <task ...>...</task> blocks that captures BOTH the opening-tag
    attribute string (so the type= selector is not lost, as it is with
    extractTaggedBlocks) and the body, returning a typed PlanTaskInfo.

  - validatePlanTaskStructure(task): branches on the task's type.
    checkpoint:human-verify requires <what-built>/<how-to-verify>/
    <resume-signal> (the canonical triple).
    checkpoint:decision requires <decision>/<options>/<resume-signal>.
    checkpoint:human-action requires <action>/<instructions>/
    <verification>/<resume-signal>.
    Unknown checkpoint:* subtypes require only the universal
    <resume-signal> (forward-compat). All other types keep the historical
    <action>/<verify>/<done>/<files> requirements unchanged.

Canonical reference: gsd-core/references/checkpoints.md. Per-type field
sets validated against the documented templates in
agents/gsd-planner.md and gsd-core/templates/phase-prompt.md.

* fix(#2444): re-resolve body-parser to 2.3.0 in lockfile (GHSA-v422-hmwv-36x6)

GHSA-v422-hmwv-36x6 (body-parser DoS via invalid limit value, low severity,
published 2026-07-20T23:23:26Z) made tests/npm-integrity-gate.test.cjs
(#3588: root workspace production tree has no advisories) fail any subsequent
npm audit --omit=dev. The advisory affects body-parser >=2.0.0 <2.3.0 pulled
transitively via @anthropic-ai/claude-agent-sdk -> @modelcontextprotocol/sdk
-> express -> body-parser@2.2.2.

express@5.2.1 already declares body-parser as ^2.2.1, so 2.3.0 is a valid
re-resolution within express's own compatibility range — no override needed.
Regenerated the lockfile via 'npm audit fix --omit=dev' which re-resolves
transitive deps within their declared ranges; package.json is unchanged.

Verified: npm audit --omit=dev reports 0/0/0/0/0 advisories; body-parser
now reads as 2.3.0 in 'npm ls body-parser --omit=dev'.

* test(#2444): close review gap-closure tests + harden type-attr charset

Orthogonal review (code-review + security-review subagents) returned APPROVE
on Standards and Spec. Per the playbook's zero-tolerance policy, address
every Low finding:

Spec gap-closures:
- AC3 verbatim: add explicit <done> and <files> regression tests for
  non-checkpoint tasks (pre-existing tests only covered <action> and
  <verify>).
- AC2: add checkpoint:decision missing <decision>, checkpoint:human-action
  missing <action>, checkpoint:human-action missing <verification> cases
  (the implementation enforces all of these; only one missing-field case
  per type was previously tested).
- Remove the duplicate 'returns error for nonexistent file' test that
  leaked into the new describe block from the insertion edit.

Security hardening (Low-sev, defense-in-depth):
- Tighten the task type= attribute extractor in src/verify.cts from
  [^"'>\s]+ to [\w:-]+ so a hostile type= attribute cannot carry
  markup fragments (e.g. type=evil<fragment) into the verifier's typed
  JSON output. All legitimate type values (auto, tracer, manual,
  checkpoint:human-verify, checkpoint:decision, checkpoint:human-action,
  checkpoint:tdd-review) match the tighter charset.
- Add adversarial regression test asserting type=evil<fragment surfaces
  as 'evil' (capture stops at '<'), with no markup chars (< > ( ) &)
  in the surfaced type field.

* docs(changeset): add Fixed fragments for #2444 PR

Two fragments:
- sturdy-jays-tumble.md: the verify plan-structure checkpoint fix
- witty-badgers-hum.md: the body-parser 2.3.0 re-resolution

PR number backfilled to 0 placeholder per CLAUDE.md 'PR Number Handling';
will backfill to the real PR number immediately after gh pr create returns.

85ed50cc4f test(#1972): consolidate 94 command/module regression tests into subject suites
Fold 94 issue-named command/module regression files into the canonical test file
that owns each subject-under-test, across 52 existing suites (state, config, frontmatter,
roadmap-parser, capability-registry, shell-command-projection-dispatch, plan-phase-drift-guard,
health-validation, runtime-converters, commands, etc.). Verbatim block-scoped describe
wrappers; 881 subtests conserved 1:1. No new test files.

Host-env pre-check (per B2): the only GSD_WORKSTREAM/GSD_PROJECT-touching destinations
(intel, planning-workspace) clear those vars hermetically, so folded CLI tests are safe.

Regenerates regression-name allowlist (222->162), ratchets file-count allowlist across
8 buckets (validate entry removed after dropping <=2), makes 34 relocated allow-test-rule
exemptions issue-ref-compliant (ADR-456; prunes 34 stale ids). Repoints CONTEXT.md +
ADR-0002/443/1235/3524 test-file references. lint:ci green.

Part of epic #1969. Closes #1972.

Fold 48 issue-named gsd-tools CLI regression files into the canonical test file
that owns each subcommand subject (state, roadmap, phase, milestone, audit, config,
router/dispatch, stats, verify, health, etc.), preserving every assertion and its
origin issue number as provenance (block-scoped describe wrappers, 299 subtests
conserved 1:1). No monolithic gsd-tools.test.cjs created — routes into 18 existing
per-subject suites.

Removes 48 tests/ files. Regenerates regression-name allowlist (271->231), ratchets
the file-count allowlist across 6 buckets (audit/milestone/phase/roadmap/state/verify),
and makes 10 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456; prunes
10 stale ids). Repoints one CONTEXT.md symptom ref and ADR-3524's parity-test ref.
lint:ci green.
