* fix(#33): add regression test asserting adaptive is reachable in settings.md UI The schema (sdk/shared/model-catalog.json) defines 5 model_profile values (quality, balanced, budget, adaptive, inherit). The settings.md AskUserQuestion previously omitted `adaptive`; the fix (two-question split, #3784) is already applied. This test locks the schema/UI contract so the gap cannot regress. feat(hub): drop mode/sdkLoader/SdkDispatchFailed (#175) (#220)
* feat(hub): drop mode/sdkLoader/SdkDispatchFailed (#175) The Command Routing Hub no longer carries dual-runtime selection. Removes `mode` and `sdkLoader` constructor parameters and the `SdkDispatchFailed` and `SdkLoadFailed` errorKind values. The Hub now routes exclusively through the CJS registry / handler resolution path. ERROR_KINDS enum shrinks from 6 to 4 values. phase-command-router.cjs updated to construct the Hub without the removed params (removes tryLoadSdk, getExecuteForCjs, sdkLoader fn, mode variable, and the post-dispatch SDK output branch). The 7 remaining family routers (init, phases, roadmap, state, validate, verify, cjs-command-router-adapter) do not use createHub directly and require no changes. The CJS↔SDK bridge (bin/lib/cjs-sdk-bridge.cjs) is unchanged and remains separately invokable; its removal is tracked in Phase 4 (#190). ADR-0012 is no longer amended in this PR — the decision is captured in ADR-0174 (which supersedes ADR-0012 entirely as part of the SDK-retirement migration). Amending a superseded ADR would be redundant noise. Tests: - Added assertions that Hub rejects/ignores `mode` and `sdkLoader` - Removed obsolete mode-selection branching tests - 57/57 local tests pass Closes #175. Part of #174 (ADR-0174). * chore(changeset): add docs-exempt marker (#175) P1.1's CommandRoutingHub work has no docs/ touchpoints — the architectural decision is captured in ADR-0174 (merged via PR #198). Per-phase ADR amendments would create noise; the SDK-retirement migration's docs land in Phase 6 PRs (#193-#196) once the relevant state is removed. fix(ci): reduce Windows test concurrency 4→2 to prevent synckit worker exhaustion on Node 24 (#173)
Under Node 24 on Windows, running node --test with --test-concurrency=4 causes 4 concurrent gsd-tools subprocesses to each spawn a synckit worker_threads worker for the SDK bridge. The 4 workers simultaneously contend on SharedArrayBuffer + Atomics.wait under Windows Defender scanning and NTFS latency, triggering OS-level resource exhaustion that kills worker processes with empty stderr before any output is flushed. The symptom: intermittent exit 1 with 0 test failures, varying affected test files per run, all sharing the pattern of invoking gsd-tools as a subprocess. Empty stderr distinguishes OS crash from gsd-tools app error (the error() path writes to stderr before exiting). Fix: platform-aware concurrency default — 2 on win32, 4 on Linux/macOS. The existing TEST_CONCURRENCY env-var override is preserved. Also adds a [stderr: (empty) exit:N] diagnostic note in helpers.cjs runGsdTools catch block so future empty-stderr crashes are visible in CI logs. Performs a synchronous mtime check before any generator reads sdk/dist/ — if the dist file is missing or older than its TS source, exits 1 with a clear actionable error naming both paths, both mtimes, and the npm run build:sdk hint. Single source of truth so all 9 generators import one function rather than duplicating the logic. See issue #168. The test creates an isolated tmpdir tree per subtest (with the real TS source copied in) and passes GSD_REPO_ROOT=<tmpdir> to the generator subprocess, so no real dist files are ever touched during subtests A or B. Subtest C (exits 0 on fresh dist) still uses the real repo tree because it needs actual compiled output to exercise the generator end-to-end, but only sets the TS source mtime (safe under concurrency) — it does not remove or rename any dist file. The "no /gsd-<cmd> hyphen form in source files" scan (test 2 of the
describe block) is skipped via test.skip. Motivation:
- Bug-3584 (2026-05-15) introduced runtime-slash.cjs, which intentionally
emits `/gsd-${token}` for skills-based runtimes. This is the correct
canonical form for runtime-persisted strings (ROADMAP.md, STATE.md,
recommended_actions, fix hints).
- Bug-2543 was last updated 2026-05-12, three days BEFORE bug-3584 landed.
The "no hyphen form" invariant was never updated to reflect the new
two-tier model.
- PR #154 first-pass: an agent misread bug-2543 and reverted the correct
`/gsd-plan-phase` to `/gsd:plan-phase` in phase-lifecycle-policy.ts:156,
breaking tests/bug-3584-runtime-slash-emitters.test.cjs. A 2nd-pass agent
reverted.
The canonical active invariant for runtime-emitter context is:
tests/bug-3584-runtime-slash-emitters.test.cjs
The remaining 4 tests in this file (commands/gsd/ existence, command
filename slug format, transformer behavior, non-command identifier safety)
are still valid and remain active — only test 2 is skipped. This avoids
vacuous-truth (a full describe-block skip would let all assertions pass
trivially).

Codex adversarial review of PR #164 [high finding]: the single content-scanning
test in bug-2543 was test.skip, leaving a vacuous test surface. The transformer/
filename unit tests remained active but no live source surface was guarded.
Fix: replace test.skip with an active scoped invariant.
Scope change:
- REMOVED get-shit-done/bin/lib/ from SEARCH_DIRS entirely. That directory is
runtime-emitter territory (runtime-slash.cjs, *.generated.cjs,
phase-lifecycle-policy.ts) and intentionally uses /gsd-<cmd> (hyphen) per
bug-3584's contract. Scanning it causes false positives that led to PR #154
first-pass incident.
- Added RUNTIME_EMITTER_EXCLUDES set documenting exactly why each file is exempt.
- Remaining SEARCH_DIRS (workflows/, references/, templates/, commands/gsd/,
agents/, hooks/) are Claude-facing source — colon form is correct there.
Verified: 9/9 tests pass, 0 skipped.

Codex adversarial review of PR #164 [high finding]: the previous CONTEXT.md
section was internally contradictory — line 616 claimed /gsd-<cmd> was globally
canonical while lines 634/640 correctly stated Claude-facing source text uses
/gsd:<cmd>. Same document; opposite claims.
Root cause: the first push of this section (2026-05-23) overstated the hyphen
form as universal, when the project has always had a two-tier model.
Fix: replace the section with an unambiguous directory-level matrix.
New structure:

* fix(117): fix Node-24 and Windows-22 CI failures in check-env
Two root causes:
1. version-manager-pin on Node 24 (mac/ubuntu/win):
The project root .nvmrc pins major 22 for local dev. When the CI
matrix runs Node 24, check-env.sh fails the version-manager-pin
check and exits 1, blocking the entire test job before any test
runs. Fix: skip the pin check when CI=true (GitHub Actions always
sets this). The pin is a local dev guard, not a gate for multi-
version matrix CI.
2. engines.node appears missing on Windows-22 (pkg_field backslash):
pkg_field() embedded PACKAGE_JSON directly into a node -e string
literal using require(). On Windows, the path uses backslashes
(D:\a\...) which are silently interpreted as JS escape sequences
inside the string, causing require() to fail silently (2>/dev/null
|| true). engines.node returns empty, triggering a spurious FAIL.
Fix: switch to fs.readFileSync + JSON.parse and normalise
backslashes to forward-slashes before embedding in the JS literal.
Also pass { CI: '' } from the bad-nvmrc unit test so the
version-manager-pin fixture test still exercises the mismatch path
even when running inside CI runners.

node.exe cannot resolve via fs.readFileSync. The previous fix embedded
the absolute PACKAGE_JSON path in the node -e string after converting
backslashes to forward-slashes, but the POSIX form produced by Git Bash
(/d/a/…) has no backslashes — so the conversion was a no-op and node
received an unresolvable path. The silent catch(e) { process.exit(0) }
swallowed the ENOENT, returning empty string for every engines.* field.
Fix: use './package.json' (relative to CWD). pkg_field() is always
called before any cd in the script so CWD === PROJECT_ROOT at call time.

* test(114): add failing regression tests for npm dependency integrity gate
Adds tests/npm-integrity-gate.test.cjs and four fixture directories under
tests/fixtures/npm-integrity/ covering:
- clean: matching lockfile and node_modules (expects exit 0)
- drift: declared vs installed version mismatch (expects exit 1)
Reproduces the ws 8.20.1 declared / 8.20.0 installed incident shape
using stable-dep@8.20.1 (package.json) vs stable-dep@8.20.0 (node_modules).
- extraneous: unlisted package in node_modules (exits 1; exits 0 with --ignore-extraneous)
- missing: declared package absent from node_modules (exits 1 regardless of flags)
Each test spawns scripts/check-npm-integrity.sh as a subprocess and asserts
on exit code first, then stderr content. Tests are RED at this commit because
the script does not yet exist.

Adds scripts/check-npm-integrity.sh, a Bash script that:
1. Runs `npm ls --all --json` at the invocation directory
2. Parses JSON output for invalid, missing, and extraneous package flags
3. Exits 1 on any finding; emits a structured report to stderr listing offenders
with both declared and installed versions for invalid packages
4. Exits 2 on tool error (npm/node not found, JSON parse failure)
5. Accepts --ignore-extraneous to suppress extraneous-only failures
6. Documents behaviour in --help output including remediation path
Workspace behaviour: the root package.json in this repo has no "workspaces"
field. npm ls runs at the invocation root and covers that tree only. The sdk/
sub-package is a separate, non-workspace package and is out of scope for a
single invocation. If workspaces are added in future, npm ls will traverse
them automatically (npm >=7).
The drift scenario (ws 8.20.1 declared vs 8.20.0 installed) is reproduced by
using an exact version pin in package.json combined with a mismatched
node_modules/package.json -- npm ls marks this as "invalid" and exits 1.
npm exits 0 for extraneous packages even though they appear in the JSON
"problems" array; this script detects them via JSON parsing regardless of
the npm exit code.

Replace npm-ls-based analysis with pure package-lock.json parsing so the
script runs correctly in CI and test environments where node_modules is not
installed. Key changes:
- Rewrite check-npm-integrity.sh parser to read package-lock.json directly
instead of spawning `npm ls --all --json`, which required node_modules on
disk and incorrectly flagged clean/drift fixtures as MISSING.
- Implement a self-contained semver satisfies() covering exact, caret, tilde,
comparison-operator, and compound ranges — no external semver package needed.
- Update extraneous fixture package-lock.json to include ghost-pkg with
"extraneous: true" so the lockfile-based detector can identify it.
- Update missing fixture package-lock.json to omit the node_modules/absent-dep
entry, making the absent-dep MISSING condition derivable from lockfile alone.
All 13 tests (clean ×2, drift ×3, extraneous ×3, missing ×3, help ×2) pass.

* fix(#114): treat transitive deps as non-extraneous in integrity gate
The extraneous check was comparing all lockfile packages against root
package.json declarations only. This caused every transitive dependency
(e.g. hono, ajv, @anthropic-ai/claude-agent-sdk-darwin-arm64) to be
flagged as EXTRANEOUS, producing false-positive failures in CI.
Only packages that npm itself marks with "extraneous: true" in the
lockfile represent genuinely unwanted packages. Transitive dependencies
installed by parent packages are valid and should be skipped.
All 13 existing tests continue to pass; the extraneous fixture still
works because it uses "extraneous: true" explicitly (npm's own marker).

"fatal: could not read Username for 'https://github.com': terminal prompts disabled"
That is a hosted-runner infrastructure flake — no code defect. The run
cannot be retried via gh CLI (too old). This empty commit kicks a fresh
full CI cycle.

The base64-scan.sh root cause is a known bash pitfall: the assignment `local printable_count=$(... | tr -cd '[:print:]' | ...)` uses `local` on the same line, which always returns exit 0, masking the tr failure. Three regex defects in cmdRoadmapAnnotateDependencies (roadmap.cjs):
1. Plans-block detection (line ~553): `Plans:\s*\n` required no text after
the colon, silently skipping `Plans: 3 plans\n` and `**Plans:** N\n`.
Fixed: `\*{0,2}Plans\*{0,2}:[^\n]*\n` + require `+` checklist lines so
a bold summary line above a bare `Plans:` block doesn't consume the match.
2. Phase-section boundary (line ~542): `\d` matched only one digit, so
`### Phase 02.3:` was not recognised as a section terminator, allowing
plan-list content from adjacent decimal phases to bleed in. Fixed with
`\d[\d.]*`. Same one-digit boundary also patched in roadmap.cjs (analyze
path), phase.cjs (insert-after path), and init.cjs (section-slice path).
3. Plan-ID extraction (line ~566): `[\w-]+?` excluded `.`, capturing `02`
from `02.3-01-PLAN.md` instead of `02.3-01`, so planData.find never
resolved and every plan defaulted to wave 1. Fixed: `[\w.-]+?`.

phaseInsert (TS) and cmdPhaseInsert (CJS) previously used a heading-only
regex (#{2,4}\s*Phase\s+N:) to locate the target phase. On projects whose
ROADMAP uses the checked-bullet format (- [ ] **Phase N: name** or
- [ ] Phase N: name), the lookup always failed with "Phase N not found".
Extend the locator to also accept the bullet form — mirroring the patterns
already used by phaseRemove and phaseComplete. When bullet-style is
detected, insert a new bullet entry after the matched line (preserving
bold/plain style to match surrounding entries). The heading-style code
path is unchanged.
Also fix a pre-existing test timeout: the first registry-integration test in
phase-lifecycle.test.ts was failing with STACK_TRACE_ERROR (masked timeout)
because the cold import of index.js takes >5 s. Added { timeout: 30_000 }.

for others — is the #3098 case where the detail section is absent;
that path must still error with "missing a detail section".
Adds a regression test (#3098 preserved) in both TS and CJS to
confirm that a heading-style ROADMAP with a bullet-only entry for
the target phase still fires the "missing a detail section" error,
not the bullet-insert path.

* fix(3407): snapshot old release into gsd-pristine, not new saveLocalPatches() was wiping gsd-pristine/ then re-populating it from pristineCtx.packageSrc — the NEW release source tree. For files that changed between old and new releases, this wrote NEW-release bytes as the pristine baseline while backup-meta.json recorded OLD-release hashes. The resulting hash mismatch triggered the #3657 verifier guard on every such file, causing it to skip the three-way diff baseline and fall back to the over-broad heuristic — effectively nullifying the #2998 feature for any file that changed upstream. Fix: preserve existing gsd-pristine/ entries that are already correct (sha256 on disk matches originalHash from manifest). These were written by the previous install with old-release bytes and remain valid. For files where no correct entry exists, leave gsd-pristine/ absent so the verifier falls back cleanly to over-broad mode — safe, never false-fails. OK_PRISTINE_DRIFT_DETECTED (added by #3657) is intentionally kept: it guards installations that already have drifted pristine from pre-fix runs and protects against other future stale-pristine scenarios. It is not removed because its guard is correct; only its trigger frequency drops. Codex skill adapter's Execute-mode-fallback section now explicitly instructs
the agent to append `--text` to `{{GSD_ARGS}}` to activate the workflow's
built-in TEXT_MODE branching. This ensures every AskUserQuestion gate is
handled consistently through the workflow's own text-mode mechanism rather
than ad-hoc plain-text fallback, and eliminates any path to silent-default
selection (#3018 / #3808).
Adds regression test bug-3808-codex-adapter-text-mode-fallback.test.cjs with
typed semantic-flag assertions covering gsd-plan-phase, gsd-discuss-phase,
gsd-execute-phase, and gsd-verify-work.

Stale-lock reclamation: on EEXIST, parse the on-disk PID and reclaim immediately if it matches process.pid (same-process re-entry, the primary #3670 failure mode) or if the PID is dead (ESRCH).
