b8c33647d8 refactor(tests): retire output-grep & source-grep via typed surfaces (finish #2974) (#462)
* refactor(#455): implement typed surfaces to retire grep tests

Production surfaces added:
- hooks/managed-hooks-registry.cjs: new CJS module exporting MANAGED_HOOKS
  as a typed array; gsd-check-update-worker.js now requires it instead of
  declaring an inline array
- bin/install.js: elevate inline gsdHooks to module-level GSD_UNINSTALL_HOOKS,
  export it alongside runtimeMap/allRuntimes (already exported)
- scripts/build-hooks.js: export HOOKS_TO_COPY; guard build() behind
  require.main===module so tests can require the file without triggering a build
- get-shit-done/bin/lib/init.cjs: add --json mode to agent-skills command,
  emitting typed IR { agent_type, block, skills_count } for test assertions
- get-shit-done/bin/gsd-tools.cjs: wire --json flag for agent-skills dispatch

Category-B source-grep migrations:
- tests/managed-hooks.test.cjs: require MANAGED_HOOKS from registry, drop fs.readFileSync+regex
- tests/orphaned-hooks.test.cjs: require MANAGED_HOOKS+HOOKS_TO_COPY as typed exports

pending-migration-to-typed-ir token cleared (87 of 87 files):
- 78 files already had source-text-is-the-product; removed duplicate token
- 5 files already used typed assertions; reclassified or annotated
- 4 files required individual reclassification to source-text-is-the-product
  or architectural-invariant

The branch commit 4e630d99 stripped `allow-test-rule: pending-migration-to-typed-ir`
from ~80 test files without replacing their assertions or adding the correct
exemption annotation. The files were NOT source-grep tests — they read .md
workflow/agent/command/reference files (source-text-is-the-product) or hook
source files for structural invariants (structural-regression-guard). No
assertion logic was changed; only the correct allow-test-rule annotation was
added to each file per CONTRIBUTING.md exception matrix.

73 files: `source-text-is-the-product` — workflow/agent/command/reference .md
7 files:  `structural-regression-guard` — hook .js / bin/install.js structural checks

33afb4f6eb chore(#452): add ESLint 9 flat-config harness with three custom AST rules (#460)
Install eslint@9 + typescript-eslint@8 + globals@16 + eslint-plugin-n@17 +
eslint-plugin-no-only-tests@3 + typescript as devDependencies.

eslint.config.mjs (flat config):
- Global ignores: node_modules, dist, .worktrees, .claude, coverage, the
  12 generated get-shit-done/bin/lib/*.cjs files
- Block for get-shit-done/bin/**/*.cjs + scripts/**/*.cjs: js.recommended +
  eslint-plugin-n + local plugin; generic quality rules (no-var, prefer-const,
  no-unused-vars, no-empty, n/no-process-exit)
- Block for tests/**/*.test.cjs: no-only-tests (error), local timing rules,
  no-restricted-syntax timing bans

eslint-rules/ local plugin (three AST rules, all at warn pending cleanup):
- no-source-grep: flag readFileSync on source .cjs/.js/.ts + text methods
- no-magic-sleep-in-tests: flag Atomics.wait and await-new-Promise(setTimeout)
- no-elapsed-assertion: flag assert*() on timing props (elapsed/duration/took/ms)

tsconfig.lint.json: allowJs + checkJs + noEmit for future type-aware passes.

tests/eslint-rules.test.cjs: 15 RuleTester unit tests (all pass, 0 fail).

package.json: add lint/lint:fix scripts; remove lint:tests (subsumed by ESLint
local/no-source-grep). Rules that produced pre-existing errors downgraded to
warn: no-useless-escape, no-unsafe-finally, no-regex-spaces, no-control-regex,
no-irregular-whitespace. ESLint exits 0 (warnings ok).

.github/workflows/test.yml lint-tests job: add npm ci + ESLint step; remove
"Lint — no source-grep tests" step (now covered by ESLint); bump timeout 3→5
min. .gitignore: add node_modules/.cache/eslint/ entry.

eslint --fix auto-cleaned: no-regex-spaces in tests, prefer-const in state.cjs,
redundant eslint-disable-next-line comments.

69a3427189 fix(3691): match all Plans-block variants in annotate-dependencies
Three regex defects in cmdRoadmapAnnotateDependencies (roadmap.cjs):

1. Plans-block detection (line ~553): `Plans:\s*\n` required no text after
   the colon, silently skipping `Plans: 3 plans\n` and `**Plans:** N\n`.
   Fixed: `\*{0,2}Plans\*{0,2}:[^\n]*\n` + require `+` checklist lines so
   a bold summary line above a bare `Plans:` block doesn't consume the match.

2. Phase-section boundary (line ~542): `\d` matched only one digit, so
   `### Phase 02.3:` was not recognised as a section terminator, allowing
   plan-list content from adjacent decimal phases to bleed in. Fixed with
   `\d[\d.]*`. Same one-digit boundary also patched in roadmap.cjs (analyze
   path), phase.cjs (insert-after path), and init.cjs (section-slice path).

3. Plan-ID extraction (line ~566): `[\w-]+?` excluded `.`, capturing `02`
   from `02.3-01-PLAN.md` instead of `02.3-01`, so planData.find never
   resolved and every plan defaulted to wave 1. Fixed: `[\w.-]+?`.

dist is absent, they threw hard errors rather than observable skips.

Apply the same `if (!existsSync(...)) { t.skip(...); return; }` guard used in
bug-2767-gsd-sdk-commit-files-flag.test.cjs (c2812313). Tests 1 & 2 guard on
sdk/dist/cli.js; test 3 guards on sdk/dist/query/config-schema.js. When dist
is present all 3 run; when absent all 3 emit actionable skip lines.

producing a flake under CI load. Redesigned using a file-barrier (Option A)
that forces both subprocesses to reach a ready-gate before either proceeds,
guaranteeing true concurrent lock contention and eliminating timing dependency.

1a1ad2ec87 Merge origin/main into feat/3597-split-suites-node-matrix
Resolves conflict in .github/workflows/test.yml: keep the 6 new drift-check
steps from main (plan-scan, secrets, schema-detect, decisions,
workstream-name-policy, Shared Module hand-sync) before the split-lane test
runs from this PR. PR's dedicated `coverage` job replaces main's per-matrix
`Run tests with coverage` step.

Other conflicting files (CONTRIBUTING.md, get-shit-done/bin/lib/init.cjs,
package.json) auto-merged cleanly. Changeset files (.changeset/*) brought in
from main as adds.

* fix(3569): surface phase_status from init.plan-phase + gate /gsd:plan-phase on closed phases

Adds a new `phase_status` field to the `init.plan-phase` SDK + CJS query
output and a §1.5 "Closed-Phase Gate" in workflows/plan-phase.md that
short-circuits on closed phases instead of silently replanning over
shipped code.

`gsd-sdk query init.plan-phase <N>` returned the same "ready to plan"
payload for a closed phase (REQUIREMENTS Met, VERIFICATION.md status:
passed, ROADMAP flipped) as for an open one. No field signaled closure,
so `/gsd:plan-phase --reviews` happily replanned over closed phases —
risking documentation drift on already-shipped code.

- Export `determinePhaseStatus` from `commands.cjs` (already present, was
  module-private).
- Both `cmdInitPlanPhase` (CJS) and `initPlanPhase` (TS SDK) now compute
  `phase_status` from plan/summary counts + VERIFICATION.md status using
  the existing `determinePhaseStatus` helper — the project-wide phase
  lifecycle vocabulary (Pending | Planned | In Progress | Executed |
  Complete | Needs Review). No directory yet → Pending.
- Workflow `plan-phase.md` adds §1.5 "Closed-Phase Gate":
  - `phase_status == "Complete"` with `--reviews` → hard-stop, no
    override (replanning a closed phase via review feedback is never
    legitimate; concerns belong in a follow-up phase or new issue).
  - `phase_status == "Complete"` without `--force` → exit with a clear
    notice pointing at VERIFICATION.md.
  - `phase_status == "Complete"` with `--force` → continue with a
    transcript banner so the deliberate replan is visible.

`Executed` and `Needs Review` are intentionally not gated — those mean
planning finished but verification did not pass, and replanning is the
correct next step.

- SDK: 4 new `phase_status` cases in init.test.ts covering Pending /
  Planned / Executed / Complete transitions.
- Existing init.plan-phase golden parity test continues to pass (the
  `researcher_model: '' vs sonnet` drift in that test predates this
  change and is unrelated).
- Full Mac+Docker suite: 9323 / 9323 passed (Mac), 9318 / 9323 passed
  (Docker, 5 skipped).

ca3be82f71 fix(3597): clear residual Windows test failures + add ratchet lint guard
Two more diagnostic passes (clusters J: residuals in already-touched files,
K: 12 untouched files) plus a production-code path fix and a new
ratchet-style lint guard.

## Test-only fixes (14 files)

bug-1736, bug-2248, bug-2698 — replace inline 1s-budget rmSync with the
shared cleanup() helper (5s budget, 20×250ms retries). The earlier
inline maxRetries:10 / retryDelay:100 wasn't enough to absorb Windows
Defender's deferred-scan handle hold on cold runners.

bug-2256, skill-manifest — also override USERPROFILE alongside HOME in
beforeEach/runGsdTools calls. os.homedir() reads USERPROFILE on win32,
so HOME-only stubs leak the runner's real home into the SUT.

bug-2784, bug-3608, enh-2500, enh-2790, few-shot-calibration,
gsd-settings-advanced — CRLF tolerance: literal \n in regexes against
file content (frontmatter anchors, bash-fence regex, multi-line
numbered-list captures, awk-block extractors) becomes \r?\n; split('\n')
becomes split(/\r?\n/). Windows checkout with autocrlf=true puts \r
before every \n; .+ doesn't match \r in JS regex by default.

bug-2966 — three-part fix to extractStepRun (CRLF split), awk regex
(\r?\n), and conflict-marker parser (rawLine + \r$ strip).

bug-2969, config — normalize separators on test assertions where the
SUT correctly emits \ on win32 but the test compares against /.

prompt-injection-scan — normalize relPath via replace(/\\/g, '/') before
ALLOWLIST.has() lookup. ALLOWLIST keys are POSIX; path.relative returns
backslashes on win32 → falsely scans allowlisted security module → trips
the boundary-tag detector on its own legitimate detection code.

prune-orphaned-worktrees — use the existing canonicalPath +
listedWorktreePaths(repoDir).has(...) helpers instead of substring
matching the raw path. git stores long-form canonical paths
(runneradmin), but mkdtempSync returns 8.3 short-form (RUNNER~1) on
Windows runners; plain string compare misses every entry.

## Production-code fix (1 file)

get-shit-done/bin/lib/init.cjs — bug-3491 in_nested_subdir computation
canonicalizes both worktreeRoot and cwd via fs.realpathSync.native +
path.relative before declaring "nested." Windows runner cwd (8.3 short
name) vs git's --show-toplevel (long form, forward slashes) made the
raw string compare always say true even at the worktree root, breaking
the "init new-project at worktree root" subtest.

## New ratchet lint guard

tests/windows-test-parity-guard.test.cjs — scans tests/ for 7
anti-patterns that drove the Windows failure clusters. Each rule has a
baseline count snapshot from this PR; the test fails when a new
occurrence appears (count grows above baseline), ratcheting down as
existing offenders are fixed. Patterns covered:

  G1 split('\n') after readFileSync (use /\r?\n/)
  G2 ```bash\n fence regex (use ```bash\r?\n)
  G3 ^---\n frontmatter anchor (use ^---\r?\n)
  G4 hardcoded "/tmp/..." literal passed to fs.* (use os.tmpdir())
  G5 bare 'npm' to execFileSync without {shell:true} on win32
  G6 process.env.HOME stub with no USERPROFILE
  G7 fs.rmSync({recursive,force}) without maxRetries

Future Windows-parity regressions get caught at PR time rather than
five iterations into a CI loop.

Validated: holodeck (ubuntu docker) 11232/0 pass (count +8 = the 7
new ratchet tests + parent describe).

034b47c8d0 fix(3584): runtime-aware slash formatter for user-facing emissions
Introduce `runtime-slash.cjs` as the single source of truth for emitting
GSD slash-command references in user-facing runtime output and persisted
artifacts. `formatGsdSlash(commandName, runtime)` produces `/gsd-<cmd>`
for skills-based runtimes (Claude/Cursor/OpenCode/Kilo/etc.) and
`$gsd-<cmd>` for Codex. The deprecated `/gsd:<cmd>` colon form is never
emitted — pasting a recommended-action command into Claude Code now
routes correctly instead of failing with `Unknown command`.

Wired into the high-impact emitters identified in #3584:

- `init.cjs` `cmdInitManager` recommended_actions[].command (the
  original failure path in the bug report) plus the no-ROADMAP /
  no-STATE error hints.
- `phase.cjs` `cmdPhaseAdd`, `cmdPhaseAddBatch`, `cmdPhaseInsert` —
  ROADMAP.md `Plans:` references now persist the routable form
  instead of the legacy colon form.
- `verify.cjs` `cmdValidateHealth` — every fix-hint addIssue() call
  (E001/E002/E003/E004/E005, W002/W003/W008/W009/W011/W016/W018) and
  the persisted STATE.md regenerate / MILESTONES.md backfill notes.
- `milestone.cjs` `cmdMilestoneComplete` — Operator Next Steps tail
  rewrite.
- `validate-command-router.cjs` — `validate context` recommendation
  strings for WARNING/CRITICAL utilization bands.
- `workstream.cjs` — missing .planning hint.
- `profile-output.cjs` — `generate-claude-md` workflow enforcement
  block, project/skills fallbacks, profile placeholder, and the
  dev-preferences refresh hints.
- `drift.cjs`, `gsd2-import.cjs`, `commands.cjs scaffold context` —
  remaining one-off persisted references.

Runtime detection: `resolveRuntime(projectDir)` reads
`process.env.GSD_RUNTIME` first, then a side-effect-free direct read of
`.planning/config.json` (NOT `loadConfig`, which would normalize legacy
keys and re-write the file just to read the runtime name).

Tests:
- `tests/bug-3584-runtime-slash-formatter.test.cjs` — 22 unit tests
  covering the pure formatter and resolver (hyphen vs codex, prefix
  normalization, defensive returns, env/config/default chain).
- `tests/bug-3584-runtime-slash-emitters.test.cjs` — 6 integration
  tests exercising `init manager`, `phase add` (via the structured
  `roadmap get-phase` payload to avoid raw-text matching on the
  on-disk artifact), `validate health`, `validate context`, and the
  codex variant.
- Existing tests updated to assert the new contract: validate-context
  recommendations, claude-md workflow block, milestone complete
  Operator Next Steps. Copilot-install engine-conversion test now
  asserts against a synthetic input since bin/lib/*.cjs no longer
  contains literal `/gsd:` references for the install-time converter
  to rewrite.

INVENTORY.md and INVENTORY-MANIFEST.json updated for the new module
(64 CLI modules shipped, +1).

0aa4bd92fb fix(3569): surface phase_status from init.plan-phase + gate /gsd:plan-phase on closed phases
Adds a new `phase_status` field to the `init.plan-phase` SDK + CJS query

`Executed` and `Needs Review` are intentionally not gated — those mean
planning finished but verification did not pass, and replanning is the
correct next step.

## Tests

- SDK: 4 new `phase_status` cases in init.test.ts covering Pending /
  Planned / Executed / Complete transitions.
- Existing init.plan-phase golden parity test continues to pass (the
  `researcher_model: '' vs sonnet` drift in that test predates this
  change and is unrelated).
- Full Mac+Docker suite: 9323 / 9323 passed (Mac), 9318 / 9323 passed
  (Docker, 5 skipped).

fb6633ceda fix(workflow): detect nested git worktree in new-project bootstrap (#3491)
The `has_git` boolean returned by `init new-project` and `init ingest-docs`
was derived from a shallow `pathExists(cwd, '.git')` check, so a subdirectory
of an existing repo reported `has_git: false`. The workflow then ran
`git init`, creating a nested `.git` inside the outer worktree and silently
diverting subsequent `gsd-sdk commit` calls into the nested repo.

Replace the shallow check with `git rev-parse --is-inside-work-tree`
semantics in both CJS (`get-shit-done/bin/lib/init.cjs`) and TS
(`sdk/src/query/init.ts`, `sdk/src/query/init-complex.ts`) handlers via a new
shared `gitWorktreeInfoInternal` helper, and expose `git_worktree_root` +
`in_nested_subdir` so the workflows can refuse `git init` inside an existing
worktree and warn that planning files will track to the outer repo.

Regression test: `tests/bug-3491-nested-git-worktree.test.cjs`.

  target without compensating benefit.
- The CI workaround pattern (cache-dependency-path: sdk/package-lock.json
  + `npm install --no-package-lock`) papered over the symptom rather

The CJS→TS port at sdk/src/query/config-mutation.ts:240,243 and
config-query.ts:122,128,132 dropped the masking layer that secrets.cjs
spec defines for brave_search/firecrawl/exa_search. Result: the SDK
echoed plaintext API keys into machine-readable JSON output (stdout,
transcripts, CI logs).

Adjacent leak in init.ts:673-675 / init.cjs:728-730: the init bundle
passed config.brave_search through raw, leaking the API key whenever
the user had stored one.

Fix:
- New sdk/src/query/secrets.ts ports SECRET_CONFIG_KEYS, isSecretKey,
  maskSecret, maskIfSecret. Exact CJS parity (verified by 17 tests
  in secrets.test.ts that import secrets.cjs and compare).
- config-set masks value + previousValue in response; on-disk plaintext
  intact (key stays usable).
- config-get masks read response. --default flows through unmasked
  (user's own input, not stored secret).
- init.ts/init.cjs mask string values only; booleans (availability
  flags) pass through unchanged so the typed contract is preserved.

Tests: 17 in secrets.test.ts (including CJS parity), 5 in
config-mutation.test.ts (#2997 block — covers on-disk-preserved,
previousValue masking, short-value, unset, non-secret pass-through),
4 in config-query.test.ts.

matched `**Requirements**:` (colon outside bold), so phases declared with
the equally-valid markdown form `**Requirements:**` (colon inside bold,
which is what the project's own templates emit) returned phase_req_ids:
null for both `init plan-phase` and `init execute-phase`.

The mirror-image bug in `phase complete`'s REQUIREMENTS.md traceability
sweep at get-shit-done/bin/lib/phase.cjs:871 only matched the inside-bold
form, silently skipping the REQ-ID checkbox flips for any roadmap that
used the outside-bold form. Both parsers now share the same canonical
regex that accepts all three rendered-identical variants:

  **Requirements:**     (colon inside bold)
  **Requirements**:     (colon outside bold)
  **Requirements** :    (space before outside colon)

Tests:
- tests/init.test.cjs — parameterized over the three header variants for
  both init plan-phase and init execute-phase (6 new behavioral cases).

  exercising initPlanPhase through the SDK.

initMilestoneOp (SDK + CJS) derives phase_count and completed_phases from
the current milestone section of ROADMAP.md instead of counting on-disk
`.planning/phases/` directories. After `phases clear` at the start of a new
milestone the on-disk set is a subset of the roadmap, causing premature
`all_phases_complete: true`.

validateHealth W002 now unions ROADMAP.md phase declarations (all milestones
— current, shipped, backlog) with on-disk dirs when checking STATE.md phase
refs. Eliminates false positives for future-phase refs in the current
milestone and history-phase refs from shipped milestones.

Falls back to legacy on-disk counting when ROADMAP.md is missing or
unparseable so no-roadmap fixtures still work.

Adds vitest regressions for both handlers; all 66 SDK + 118 CJS tests pass.

* fix(#2633): preserve full phase tokens in W002 + completion lookup

CodeRabbit flagged that the parseInt-based normalization collapses distinct
phase IDs (3, 3A, 3.1) into the same integer bucket, masking real
STATE/ROADMAP mismatches and miscounting completions in milestones with
inserted/sub-phases.

Index disk dirs and validate STATE.md refs by canonical full phase token —
strip leading zeros from the integer head only, preserve [A-Z] suffix and
dotted segments, and accept just the leading-zero variant of the integer
prefix as a tolerated alias. 3A and 3 never share a bucket.

Also widens the disk and STATE.md regexes to accept [A-Z]? suffix tokens.

a6e692f789 fix(#2646): honor ROADMAP [x] checkboxes when no phases/ directory exists (#2669)
initProgress (and its CJS twin) hardcoded `not_started` for ROADMAP-only
phases, so `completed_count` stayed at 0 even when the ROADMAP showed
`- [x] Phase N`. Extract ROADMAP checkbox states into a shared helper
and use `- [x]` as the completion signal when no phase directory is
present. Disk status continues to win when both exist.

Adds a regression test that reproduces the bug with no phases/ dir and
one `[x]` / one `[ ]` phase, asserting completed_count===1.

73c1af5168 fix(#2543): replace legacy /gsd-<cmd> syntax with /gsd:<cmd> across all source files (#2595)
Commands are now installed as commands/gsd/<name>.md and invoked as
/gsd:<name> in Claude Code. The old hyphen form /gsd-<name> was still
hardcoded in hundreds of places across workflows, references, templates,
lib modules, and command files — causing "Unknown command" errors
whenever GSD suggested a command to the user.

Replace all /gsd-<cmd> occurrences where <cmd> is a known command name
(derived at runtime from commands/gsd/*.md) using a targeted Node.js
script. Agent names, tool names (gsd-sdk, gsd-tools), directory names,
and path fragments are not touched.

Adds regression test tests/bug-2543-gsd-slash-namespace.test.cjs that
enforces zero legacy occurrences going forward. Removes inverted
tests/stale-colon-refs.test.cjs (bug #1748) which enforced the now-obsolete
hyphen form; the new bug-2543 test supersedes it. Updates 5 assertion
tests that hardcoded the old hyphen form to accept the new colon form.

f101a5025e fix(map-codebase): pass current date to mapper agents to fix wrong Analysis Date (#2298)
The `cmdInitMapCodebase` / `initMapCodebase` init handlers did not
include `date` or `timestamp` fields in their JSON output, unlike
`init quick` and `init todo` which both provide them.

Because the mapper agents had no reliable date source, they were forced
to guess the date from model training data, producing incorrect
Analysis Date values (e.g. 2025-07-15 instead of the actual date) in
all seven `.planning/codebase/*.md` documents.

Changes:
- Add `date` and `timestamp` to `cmdInitMapCodebase` (init.cjs) and
  `initMapCodebase` (init.ts)
- Pass `{date}` into each mapper agent prompt via the workflow
- Update agent definition to use the prompt-provided date instead of
  guessing

e6e33602c3 fix(init): ignore archived phases from prior milestones sharing a phase number (#2186)
When a new milestone reuses a phase number that exists in an archived
milestone (e.g., v2.0 Phase 2 while v1.0-phases/02-old-feature exists),
findPhaseInternal falls through to the archive and returns the old
phase. init plan-phase and init execute-phase then emitted archived
values for phase_dir, phase_slug, has_context, has_research, and
*_path fields, while phase_req_ids came from the current ROADMAP —
producing a silent inconsistency that pointed downstream agents at a
shipped phase from a previous milestone.

cmdInitPhaseOp already guarded against this (see lines 617-642);
apply the same guard in cmdInitPlanPhase, cmdInitExecutePhase, and
cmdInitVerifyWork: if findPhaseInternal returns an archived match
and the current ROADMAP.md has the phase, discard the archived
phaseInfo so the ROADMAP fallback path produces clean values.

Adds three regression tests covering plan-phase, execute-phase, and
verify-work under the shared-number scenario.

In cmdPhaseComplete, backlog phases (999.x) on disk were picked as the
next phase when intervening milestone phases had no directory yet. Now
the filesystem scan skips any directory whose phase number starts with 999.

In cmdInitManager, all_complete compared completed count against the full
phase list including 999.x stubs, making it impossible to reach true when
backlog items existed. Now the check uses only non-backlog phases.

Closes #2129

d59d635560 feat: add gsd-pattern-mapper agent for codebase pattern analysis (#1861)
Add a new pattern mapper agent that analyzes the codebase for existing
patterns before planning, producing PATTERNS.md with per-file analog
assignments and code excerpts. Integrated into plan-phase workflow as
Step 7.8 (between research and planning), controlled by the
workflow.pattern_mapper config key (default: true).

Changes:
- New agent: agents/gsd-pattern-mapper.md
- New config key: workflow.pattern_mapper in VALID_CONFIG_KEYS and CONFIG_DEFAULTS
- init plan-phase: patterns_path field in JSON output
- plan-phase.md: Step 7.8 spawns pattern mapper, PATTERNS_PATH in planner files_to_read
- gsd-plan-checker.md: Dimension 12 (Pattern Compliance)
- model-profiles.cjs: gsd-pattern-mapper profile entry
- Tests: tests/pattern-mapper.test.cjs (5 tests)
