enhance(#1577): WebFetch/WebSearch injection isolation + opt-in blocking (#1585)
* fix(#1577): isolate WebFetch/WebSearch ingress + opt-in injection blocking Split A of #1573 (security-critical). Scans WebFetch/WebSearch output (the largest untrusted channel) in gsd-read-injection-scanner; shared untrusted-input-boundary reference @-included by the 8 ingest agents (randomized per-wrap delimiters, in-prompt self-scan guard, task-anchoring); opt-in security.injection_blocking (default advisory — non-breaking). arXiv: 2506.05739 (PPA), 2507.15219 (PromptArmor), 2504.20472 (Referencing), 2503.00061 (defense-in-depth). * fix(#1577): address review — honest blocking docs, config key, ADR, property test, revert localized - A1: rewrote the opt-in-blocking doc + Security changeset honestly — the PostToolUse hook is a circuit-breaker (halts the agent's next step), NOT a redactor; it does not scrub content already in the transcript. The prompt-level data/instruction boundary is the primary control. - A2: registered security.injection_blocking in the config schema + defaults manifests (default false) + an e2e config-roundtrip test; the dotted setter writes the nested shape the hook reads. - A3: reverted the 4 hand-edited localized security-model.md (canonical EN only, per convention). - A5: ADR-1577 (untrusted-input boundary + opt-in blocking; redaction-vs-circuit-breaker rationale). - A6: property test — scanner never crashes / only emits valid JSON on unicode/large/malformed input. - Also: inventory (untrusted-input-boundary.md) + agent-size baseline (8 ingest agents) + drift-guard matcher update (Read -> Read|WebFetch|WebSearch). A7 (content<20 early-exit) left as the noted pre-existing follow-up. * fix(#1577): allowlist untrusted-input-boundary.md in injection-scan CI gate The new reference quotes injection phrases ('ignore previous instructions', 'you are now…') as examples agents must NOT comply with, tripping the repo's own prompt-injection-scan.sh diff gate (the standalone 'security' CI job, red on HEAD). Allowlist it alongside the other security docs (security-model.md, TEST-EXAMPLES.md) that legitimately demonstrate injection patterns. The JS scanner test doesn't scan references/, so only the shell gate needed it. Verified: scan --diff origin/next -> 0 findings; scanner JS test 15/15. * fix(#1577): cover AC #2's gsd-ui-researcher + gsd-assumptions-analyzer trek-e Major 1: the @-included set dropped two AC #2 agents. Restore them so no named web-ingress agent is uncovered, keeping the two justified additions (gsd-ai-researcher, gsd-domain-researcher). Final set = AC's 8 + 2 = 10. - gsd-ui-researcher carries the full WebSearch/WebFetch + MCP-fetch toolset. - gsd-assumptions-analyzer reads 5-15 codebase source files (external/source- document ingress per the boundary), though it has no web tools. INGEST_AGENTS in the isolation test now asserts all 10; size baselines regenerated (+60 bytes each, both well under the DEFAULT cap); changeset reworded 8 -> 10. Verified: untrusted-input-isolation 14/14; agent-size-budget 39/39. * docs(#1577): document security.injection_blocking + boundary seam trek-e Major 2 + Minor: - docs/CONFIGURATION.md: add the top-level security.injection_blocking key to the Full Schema and a Security Settings subsection, distinguishing it from the workflow.security_* namespace; honest circuit-breaker-not-redactor framing matching ADR-1577 / security-model. - CONTEXT.md: add the 'Untrusted-input boundary' seam glossary entry. Verified: lint:docs ok; config-field-docs + contributor-standards green. * test(#1577): make read-injection property test git-text, not binary trek-e nit (and more): the file embedded a raw U+FFFF AND a raw NUL byte as degenerate-edge inputs. The NUL is what actually made git classify it binary (git binary = NUL in first 8K). Replace both with text-safe escapes that keep the identical runtime values: '\\x00' and String.fromCodePoint(0xFFFF). File now diffs/blames line-by-line. fix(#1626): make the security gate severity-aware via per-threat severity (#1635)
workflow.security_block_on was documented as the minimum threat severity that blocks advancement, but threats carried no severity and the auditor's threats_open count (the SECURITY.md gate field) counted every open threat regardless of severity — so the threshold had no effect, and the auditor's block_on vocabulary (open/unregistered/none) did not even match the config enum (critical/high/medium/low/none). - planner: add a Severity column to the STRIDE threat register; assign severity per threat. - auditor: read severity; reconcile the <config> block_on domain to the severity enum; redefine threats_open as the count of OPEN threats whose severity is at or above block_on (none => 0). Below-threshold opens are reported as non-blocking and excluded from threats_open. - SECURITY.md template + planning-config.md reconciled. workflow.security_asvs_level was display-only — the planner hardcoded 'mitigate if ASVS L1 requires it' and the auditor only echoed the level, so L2/L3 behaved identically to L1. - New reference gsd-core/references/security-asvs-levels.md defines L1 (opportunistic), L2 (standard), L3 (comprehensive) for both planner threat disposition and auditor verification depth (higher = superset). - planner: disposition now scales with the configured ASVS level (no hardcoded L1) + @-pointer to the reference. - auditor: verification depth scales with asvs_level (L1 grep-presence, L2 boundary/vector check, L3 end-to-end trace + bypass check). - planning-config.md + INVENTORY updated; planner kept under its 48K cap by extracting the goal-backward worked example to planner-guidance.md. enhance(verify-phase): node-test causation control — prove the RED is content-caused (#1346)
The #1279 node-test machine-proof confirmed a known-bad subject drives the negative test RED, but could not distinguish a genuine content-violation from a deceptive test that reds merely because GSD_PROHIB_SUBJECT is set. Add an optional fifth flat scalar `check_clean_fixture` (-> CheckDescriptor.cleanFixture) threading a KNOWN-CLEAN control subject through projectProhibitions + descriptorFromProjection. When present, the prover also runs the check against the clean subject and requires GREEN, so fail-first is proven only when the check is RED on the violation AND GREEN on the clean subject (content-dependent). Opt-in and additive: absent a clean fixture the prover behaves exactly as post-#1314 (no control, documented residual), preserving the zero-authoring compose path; the lint-rule kind needs no analog (its subject IS the linted file, no env indirection). Coverage: RED-first deceptive case, positive, missing-clean fail-closed, round-trip read-back/emit, fast-check property extended to the 5th scalar, and an end-to-end COMPOSE capstone (honest vs deceptive). * fix(#1369): refresh wave manifest and re-check base before each wave in execute-phase Two compounding issues caused wave N+1 worktrees to fork from the stale pre-wave-N commit, immediately tripping the worktree_branch_check FATAL guard in every executor: 1. worktree.base-check auto-degrade only ran once at initialize time. After wave N merges advanced orchestrator HEAD past origin/HEAD, new worktrees were still forked from origin/HEAD (Claude Code "fresh" base). 2. WAVE_WORKTREE_MANIFEST was never unset between waves, so wave N+1 reused the consumed wave-N manifest file, which would have blocked the step 5.5 manifest guard (#3384) on subsequent waves. Fix: add two safeguards in execute-phase.md — - Step 0.5 (start of each wave): re-runs worktree.base-check; auto-degrades USE_WORKTREES=false for that wave when HEAD has diverged from origin/HEAD. - Step 7c (end of each wave): unsets WAVE_WORKTREE_MANIFEST so wave N+1 creates a fresh per-wave manifest; re-asserts worktree.set-baseref (idempotent) and re-evaluates base degradation after wave merges land. 17 regression tests added in tests/bug-1369-wave-stale-base.test.cjs. #2551/#3182/#2361 are pre-migration get-shit-done-redux issue numbers with no equivalent in open-gsd/gsd-core; they mislead triage and manufacture phantom blockers. Repoint to real successors (#717 byte-budget rework, #720) or rewrite as prose referencing the discuss-phase/modes progressive-disclosure split. Correct co-located 'line budget'/'<500 lines' framing to the byte-based reality (#717). Add a CI guard (tests/no-phantom-issue-refs.test.cjs) that fails if a phantom ref is reintroduced. SSH-key patterns (id_ed25519) left untouched. No user-facing runtime behavior change. Closes #1073 |

Delivers option (a) from the #1314 maintainer review: thread a fourth flat scalar check_violation_fixture through the projection so a prohibition authored at spec-phase machine-proves fail-first and greens through the deterministic path alone — zero hand-authoring at verify time. - src/probe-core.cts: Prohibition gains check_violation_fixture?; projectProhibitions emits it (both kinds) ONLY for a well-formed descriptor and ONLY when non-empty (blank/absent -> projects absent -> producer hard-gates, never a partial green). - src/prohibition-enforcement.cts: descriptorFromProjection reads it back into violationFixture via the same numeric-coercion-safe scalar() normalizer. - Tests (RED-first, proven non-vacuous by reverting both src edits): CHK-02(#1346) projection emit, CHK-08(#1346) read-back, CHK-03(D) example round-trip, the fast-check round-trip property extended to the 4th scalar (the contract trek-e blocked #1301 on), and a real-subprocess COMPOSE capstone greening end-to-end through project -> descriptorFromProjection -> default prover+runner. - Docs flipped from 'hard-gates until #1346' to 'composes end-to-end': verify-phase.md, prohibition-probe.md, spec-phase.md authoring, ADR-550 addendum, changeset. #1346 now tracks only the node-test causation residual. 190 affected-suite tests green; eslint + tsc clean; size baseline regenerated. |

Addresses the #1314 maintainer review (trek-e): - Major 1 (fail-OPEN): defaultProveFailFirst's node-test branch only guarded `if (!fixture)`. A missing/typo'd/stale violationFixture made GSD_PROHIB_SUBJECT point at a missing file; an honest negative test threw ENOENT *inside its callback* (a failing test named distinctly from the file), which isNonVacuousNodeTestRed accepted as proof -> a green forged from a setup crash. Now requires fs.existsSync(path.resolve(cwd, fixture)) before spawning, symmetric with the lint-rule path's file-result guard. Regression test pins it (RED without the guard); a second test pins cwd-relative fixture resolution. - Major 2 (misleading prose): the #1278 projection carries no violationFixture, so the deterministic-locate path always hard-gates (fail-closed) until a check_violation_fixture scalar is threaded through. verify-phase.md and prohibition-probe.md no longer read as if the projected path produces greens; the ADR-550 addendum records both items. Tracked as follow-up #1346. - Documented residual: existence is necessary but not sufficient (a red caused by the env being set vs the subject's content); recorded as a constraint, in #1346. - Nit: stale 'NOT attested fail-first' comment -> 'NOT machine-proven fail-first'. 64 tests pass; eslint + tsc clean; changeset valid. |

Adversarial pre-submission review found the injected-runCheck tests masked a non-functional real runner. Fixes: - BL-01 (false green on vacuous test): the node-test runner now parses the TAP summary and requires a NON-VACUOUS pass (>=1 test, >=1 pass, 0 fail) AND a reported test named distinctly from the file — node --test counts an empty file as one passing test, so counts alone could not catch it. - SF-01 (lint anchor never greened): the lint-rule runner now runs the project eslint as --format json and filters by ruleId, so plugin rules (local/*) load via the flat config — bare --rule cannot load a plugin. local/no-source-grep now genuinely greens (covered by a real, non-injected test). - BL-02 (tautological fail-first): the runner no longer echoes the caller's failFirst as if confirmed. failFirst is documented as caller-ATTESTED; the producer requires attestation + a genuine non-vacuous pass. Rebased onto current next and relocated the whole feature from get-shit-done/ to
gsd-core/ per #615 (trek-e re-review #4, option 1). The artifact now builds to
gsd-core/bin/lib/edge-probe.cjs; all hard-coded path strings (tests, workflow
@-refs, run-tests.cjs sentinel, eslint ADR-457 ignore, .gitignore) updated.
Content conflicts in .gitignore / eslint.config.mjs / run-tests.cjs resolved.
Feature: Step 5.5 edge-completeness probe walks each SPEC requirement against a
closed 8-category edge taxonomy, proposes applicable candidate edges, and resolves
each (covered/dismissed/backstop/unresolved). covered/backstop criteria are lifted
by plan-phase into must_haves.truths, extending the goal-backward verifier's reach
to boundary edges no requirement was written for. Engine authored as strict TS
(src/edge-probe.cts, ADR-457), compiled to the gitignored gsd-core/bin/lib/edge-probe.cjs.
Folds in every prior review round on PR #584:
- RR-01..03: plan-phase resolves the phase *-SPEC.md and injects {SPEC_PATH} into the
planner; must_haves<->Edge-Coverage quality_gate; held-out planner-contract test.
- RR-04/11: Step 5.5 invokes the compiled engine at runtime (npm --prefix-pinned,
source-checkout-gated build fallback) instead of LLM re-derivation; the engine
capture is exit-checked and the report JSON-validated before use (fail closed).

resolution; Array.isArray(shapes); duplicate-resolution rejection; CLI JSON exit(2);
per-artifact build sentinel.

A proposed item with no matching author resolution is rolled up VERBATIM, but its own
status/fields were never validated -- an item carrying an out-of-enum status (the dropped

proposes `unresolved` items, but the prohibition adapter (#644) proposes LLM-generated
items that arrive populated. An Item is structurally a superset of a Resolution, so reuse

templates/planner-subagent-prompt.md is loaded by nothing at runtime (no @-import in
agents/gsd-planner.md; plan-phase.md spawns the planner from its own inline
<planning_context>), so the precise covered/backstop -> must_haves.truths lift instruction
this PR added there never reached the planner -- and the RR-02 contract test asserted it in
that dead file, giving false green. Move the instruction into plan-phase.md's runtime
<downstream_consumer> block (where the rest of the wire already lives), revert the dead-template
edit, and retarget RR-02 to the loaded surface with a guard against re-orphaning.
* test(#550): lock machine<->SPEC vocabulary mapping against drift (m2)
The machine contract uses orthogonal status x verification; the SPEC table renders a flat
covered/dismissed/backstop/unresolved. The migration map (ADR-550 Decision 7a) was prose-only
with no test, so the layers could silently drift. The SPEC table is LLM-rendered (no JS
renderer to round-trip), so pin the canonical bijection as code AND ground it in every doc
surface that renders the vocabulary (ADR migration clause, spec.md legend, reference mapping
table) -- a rename or remap now fails the suite.
* docs(#550): add how-to for resolving edge-coverage findings (B1)
Feature shipped reference coverage (FEATURES.md, COMMANDS.md, references/edge-probe.md) but
no how-to -- reference-only does not satisfy the Diataxis docs standard for a user-facing
capability. Add a single-mode how-to (imperative, goal-directed) walking each resolution
state (specify/dismiss/backstop/defer), the soft gate, and --auto, with taxonomy/concepts
linked out to the reference. Register it in the docs/how-to index.
* docs(#550): add Probe Core + Edge Probe glossary entries to CONTEXT.md (N1)
trek-e re-review #7 N1 (Major): adding probe-core/edge-probe as src/*.cts-derived
seam modules (ADR-550 Decision 7) requires CONTEXT.md Domain-terms glossary entries

(applicable === resolved + unresolved), byVerification sums ≤ resolved, per-tier
recount + resolved-status-only counting, rollup determinism, and stable orphan
rejection. 200 runs/seed 42 via helpers/fast-check-setup.cjs.

trek-e re-review #7 N3 (Nit): the // allow-test-rule: tokens (source-text-is-the-product,

'runtime-contract-is-the-product' (RULESET.TESTS.no-source-grep.exemption, CONTEXT.md:240).
All three tests assert deployed runtime-contract surfaces (spec-phase.md Step 5.5, the
plan-phase.md planner prompt, the rendered reference/SPEC/ADR vocabulary), so the canonical
category fits; each now carries a one-line justification per the ruleset format. Free-text
reason — lint behavior unchanged.

Convert the planner's soft comment-text guideline into a plan-write-time HARD GATE. When an acceptance criterion negative-greps for a literal (`grep -c 'LIT' file == 0`) and that same literal appears verbatim in an `<action>` body (JSDoc samples, head-comment references, "what NOT to do" snippets), the executor's commit-time verify gate later fails on the comment echo rather than a real regression — wasting cycles and training the executor to distrust the gate. `verify.plan-structure` (the `validate_plan` step) now scans for this: - confidently-extracted (quoted) negative-grep literal echoed in an <action> → error (valid:false), failing plan creation - unquoted/ambiguous grep target → warning (fallback policy) - `<!-- planner-discipline-allow: LIT -->` escape hatch skips a literal - positive-count gates (`== N`) and `!= 0`/`>= 0` are out of scope Adds the `<comment_text_discipline>` block to gsd-planner.md, the full rules + allowlist example to planner-antipatterns.md, and regression fixtures for downstream incidents 12-04, 11-04, 12-02 (plus a boundary case proving positive-count gate 11-02 is not flagged). On a shim-only install — where gsd-tools.cjs exists under the runtime home but gsd-tools is NOT on PATH — those calls fail with "command not found" and the agent silently skips init/state/validate/commit ceremony, deferring to the orchestrator or bypassing GSD bookkeeping entirely. were never migrated, so it persisted on Claude Code and every other runtime that consumes the source agents directly. Only gsd-phase-researcher.md carried a resolver — and a stale, claude-only truncated one. Fix (all runtimes): - Inject the canonical multi-runtime gsd_run preamble (byte-equal to _runtime-launcher.snippet.sh — claude/codex/cursor/gemini/copilot/windsurf/ augment/trae/qwen/cline/opencode/kilo/hermes/antigravity homes) at the top of the first gsd_run block of all 12 gsd-tools-calling agents, and rewrite every command-position bare gsd-tools to gsd_run. - Upgrade gsd-phase-researcher.md's stale resolver to the canonical one. - Extend scripts/sync-runtime-launcher.cjs to maintain agents/ in parity (the sync caught and corrected a mis-placed preamble during development). - Extend the bare-gsd-tools (#2851) and launcher-parity (#373) regression guards to agents/ so no runtime can silently regress. * fix(#705): route hardcoded $HOME gsd-tools invocations in agents/commands through gsd_run The hardcoded `node "$HOME/.claude/gsd-core/bin/gsd-tools.cjs" <cmd>` form (fixed for workflows in #621/#637) survived in agent/command surfaces and misresolves on global/shim-only installs. Route every agent-executed invocation through the resolved `gsd_run` launcher in gsd-phase-researcher, gsd-planner (load_graph_context extracted to a shared reference to stay under the planner size budget), import, and graphify. Add a regression guard over agents/ + commands/ + gsd-core/references/ bash blocks. User-facing display messages and docs are intentionally left untouched. * chore(#604): rename get-shit-done/ runtime directory to gsd-core/ Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary (`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers are unaffected. Mechanical (bulk, ~90% of the diff): - `git mv get-shit-done gsd-core` - Swept path/identifier references across the repo via `perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead preserves the five legitimate slug variants that are NOT the directory: get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names). - Build/manifest wiring: package.json (bin, files, coverage globs), tsconfig.build.json (outDir), ~86 .gitignore build-output entries, stryker.config.mjs, scan-ignore files, install.js path strings. - Frozen (not rewritten): CHANGELOG.md history; translated docs (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/). New logic (review here): - src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper ADR-0008 installer migration. On upgrade it walks the legacy `~/.claude/get-shit-done/` tree, classifies each file via the prior install manifest, and emits remove-managed / backup-and-remove for managed files while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked root and symlinked entries; bounds-checks every path under configDir). The framework rolls back on install failure. Emptied dirs may remain (framework has no recursive dir-removal primitive) — documented. - scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare `get-shit-done` directory token (split token to avoid self-match; case- insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines). Wired into the lint-tests CI job. - Restored scripts/lint-package-identity-drift.cjs detection regexes (the mechanical sweep had wrongly rewritten the old-name patterns it exists to detect) and marked them as intentional legacy references. - TDD tests for the migration and the guard; do.md slash-command guard regex tightened so a `/gsd-core/bin` path segment is not mistaken for a command; changeset + docs/installer-migrations.md row added. Breaking: the installed runtime path moves `~/.claude/get-shit-done/` -> `~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed files (preserving user files) on upgrade. Users with custom hooks/configs hardcoding the old path must update them.
