4918c62d76 feat(#2845): require provenance for UI-SPEC component inventories (#3745)
* test(#2845): failing-first suite for UI-SPEC inventory provenance

Binds two shared formats before either exists, so the suite is RED against
next: the gsd-ui-checker dimension roster (asserted independently on twelve
surfaces, eight English and four translated) and the provenance-line grammar
the UI-SPEC template emits and Dimension 7 consumes.

Every parity assertion is paired with a synthetic mutation case, so the guard's
failure branch executes rather than only reading a correct tree: limit-1 (a
surface still declaring 6), limit (7), limit+1 (8), a dropped dimension, a
label that drifts on one surface only, a non-contiguous roster, a duplicated
number, and a surface that stops declaring a count at all. A seeded fast-check
property renders the roster under formatting noise (CRLF, padding, interleaved
sections) and asserts the parse round-trips and is strictly sensitive to a
dropped heading.

Assertions are on parsed typed records, never raw substrings.

* docs: normalize design-a-ui-phase how-to to American English

House style for docs/ is American English (CLAUDE.md). This file carried
colour/initialisation/initialise/artefact throughout. Spelling only — no
content change; kept separate from the #2845 feature commit so the
release-notes classifier and the hotfix cherry-pick filter see it for what
it is.

* feat(#2845): require provenance for UI-SPEC component inventories

A UI-SPEC's component inventory was treated downstream as a closed allowlist
while the document recorded nothing about whether the list had been enumerated
from the installed design system or recalled from memory. A recalled inventory
is indistinguishable from an enumerated one, so an executor complying with the
spec builds against a fraction of what the package offers, and every gate stays
green because they assert semantics rather than composition.

The UI-SPEC template gains a Component Inventory slot carrying one of two
provenance lines: the command that enumerated the list, the count it returned,
the resolved package@version and the date; or a Could not enumerate record with
a real reason. gsd-ui-researcher gains an enumeration ladder and must record
the line rather than write the list from recall.

gsd-ui-checker gains Dimension 7. An inventory with no provenance line, a count
with no command, an empty could-not-enumerate reason, or a line still carrying
the template's unfilled placeholders BLOCKs; a partial line, a line placed below
its table, or an honest negative record FLAGs; a complete line passes, and so
does a spec carrying no inventory at all, which keeps every UI-SPEC predating
the dimension validating unchanged. Whatever the verdict, an unsourced inventory
is reported as a non-exhaustive list of known-good components rather than a
closed allowlist, so the executor is never blocked from a component the spec
merely failed to mention. The checker never runs the recorded command.

The dimension count moved on all thirteen surfaces that assert it, across five
languages. Also corrects the claim in the English, Korean and Portuguese how-tos
that this checker applies a scored six-pillar rubric — that rubric belongs to
/gsd-ui-review's retroactive audit.

9a69a86f42 enhance(#2971): strict planning filter mode for /gsd-pr-branch (#3720)
* test(#2971): failing-first suite for the pr-branch planning-path filter

Binds the not-yet-built planning.pr_strict mode and the corrected filter recipe
for /gsd-pr-branch across six layers: pure classification and forbidden-path
predicates, real-git fixtures that run the cherry-pick filter loop end to end,
config-key registration through the real CLI and both manifests, the executed
worktree-materialization claim the issue's triage asked to establish, fast-check
properties over arbitrary path sets, and a drift guard over the shipped workflow.

Two live defects in today's shipped recipe are pinned as regressions, both
reproduced empirically first: `git rm -r --cached` stages a deletion of any
.planning/ path the target branch already tracks, so the generated PR removes the
base branch's planning files; and the same command leaves the cherry-picked file
untracked on disk, so a second commit touching that path aborts the pick with
"untracked working tree files would be overwritten" and every remaining commit is
silently dropped.

The test helper parses the canonical path lists out of gsd-core/workflows/pr-branch.md
rather than restating them, so the workflow stays the single source of truth and the
suite cannot drift from what ships.

* feat(#2971): strict planning filter mode for /gsd-pr-branch

Adds planning.pr_strict — a boolean, default false, that selects what
/gsd-pr-branch means by "filtered". Default mode is unchanged: structural
planning state survives into the PR branch and the nine transient
subdirectories do not. Strict mode drops every .planning/ path, structural
files included, and carries a commit over only when it touches at least one
file outside .planning/.

Strict mode is what makes planning.commit_docs: true safe for a project that
versions its planning tree locally but publishes none of it. The alternative
posture, commit_docs: false, silently costs parallel executor isolation — a
worktree is checked out from a commit, so an untracked or ignored .planning/
is simply absent inside it and the executor has no PLAN.md to read. That claim
is now established by an executed fixture rather than inherited.

The two path lists are declared once and both projections derived from them,
so create_pr_branch and verify can no longer disagree about what the filter
promised. verify previously counted every .planning/ path against a documented
success criterion of zero while create_pr_branch was specified to preserve five
structural files, so a correct run reported itself as failed on every phase
that touched STATE.md — which is every phase. It now asserts against the active
mode, and names the .planning/ paths default mode deliberately keeps rather
than trading a wrong signal for silence.

Two verified defects in the same recipe are fixed alongside, because strict
mode would have amplified both. `git rm -r --cached` staged a deletion for any
.planning/ path the target branch already tracked, so the generated PR removed
the base branch's planning files — under strict mode that would have been the
entire tree. The same command left the picked file untracked on disk, so a
second commit touching that path aborted the cherry-pick with "untracked
working tree files would be overwritten" and every remaining commit was
silently dropped. Both were reproduced against real git before being fixed.
The filter now forces excluded paths back to what the PR branch's HEAD carries,
in the index and the working tree; a conflict outside the filter halts instead
of being improvised past; a commit left empty by filtering is skipped rather
than failing. A clean-working-tree precondition makes the worktree half safe.

Closes #2971

* fix(#2971): unwind the checkout on a conflict halt, and test the real recipe

Two review findings, both fixed in place.

The isolated adversarial pass found that the conflict-outside-the-filter branch
exited while leaving the user checked out on the half-built PR branch with
cherry-pick state still live — this loop runs in the user's own working
directory, so stranding them there is a real cost even though it is not a
vulnerability. The branch now aborts the pick, returns to the original branch,
removes the partial PR branch, and says so before exiting.

The standards pass found the L2 fixtures executed a hand-written mirror of the
cherry-pick filter recipe rather than the recipe itself, so a reordering in the
workflow would not have been caught — and the order is load-bearing, since
restoring a path from HEAD before removing it inverts the filter. The helper now
extracts the canonical loop from the shipped workflow and the fixtures execute
that verbatim, which also gives the conflict-halt unwind above real coverage.
The drift guard additionally pins the two commands' relative order and asserts
the workflow carries exactly one canonical loop.

Also records the publication gate in the CONTEXT.md glossary next to the commit
gate it is distinct from.

The remote matrix caught two defects in the previous commit.

The halt path claimed to restore the original branch but did not. `git
cherry-pick --abort` does not apply to a single `--no-commit` pick with no
sequencer file, and the fallback left the unmerged index in place, which makes
`git checkout` refuse — a failure the `2>/dev/null || true` then swallowed, so
the user was told they had been restored while still sitting on the half-built
PR branch. The unwind now drops sequencer state, hard-resets the disposable PR
branch to clear the unmerged index, and only claims a restore when the checkout
actually succeeded; when it does not, it says where the user is and gives them
the two commands to finish it by hand. Verified against real git: exit 1, the
conflict named, HEAD back on the original branch, the partial branch gone, a
clean tree and no CHERRY_PICK_HEAD.

Two runtime-loaded source artifacts used the retired `/gsd-<cmd>` hyphen form,
which names a command no runtime registers. The canonical authoring token for
workflows and references is `/gsd:<cmd>`; docs keep the hyphen form, so the
documentation added in this branch is unaffected. The comment in src/config.cts
moves to the colon form too, since it propagates into the generated lib.

14679b866b enhance(#2856): add default-off live-DOM UAT capability (#3716)
* test(#2856): add failing-first suite for the live-dom-uat capability

Binds the approved triage shape before any of it exists:

- containment — the execute:wave:post hook must not render unless
  workflow.live_dom_uat is true AND the capability resolves active
  (fail-closed on a missing state entry, and on a non-boolean value)
- criterion 4 — agents/gsd-executor.md carries no browser MCP family;
  asserted as an absence, which is the only way it is observable
- Hyrum guard — the pre-existing mcp__playwright__* branch must stay
  outside the key-gated block, or upgrading silently removes working
  automated UI verification for every current Playwright-MCP user
- parity — the browser glob list now lives in two surfaces (agent
  frontmatter + workflow detection block); the assertion fails if
  either gains or loses a family without the other

Red by construction: the capability, agent and workflow block do not
exist yet. Verified on the remote runner.

* enhance(#2856): add default-off live-DOM UAT capability

A phase whose acceptance criteria needed a live DOM could not be
finished by the agent that executed it: gsd-executor carries no browser
tools, so it correctly returned checkpoint:human-action even though the
work was not human-only, just tool-less. Every such phase degraded to
"executed, then finished by hand in the orchestrator", and autonomous:
false could not distinguish "a human must judge this" from "the executor
lacks the tool".

Implements the shape approved at triage, not the one reported. The
executor's tools: line is NOT widened, in any configuration: for a
first-party agent the static list is the only control that exists
(ADR-1244 D2, ADR-857 D4, no per-dispatch override). Instead one
default-off capability owns the key, the agent, and the step:

- capabilities/live-dom-uat/ — activationKey workflow.live_dom_uat
  (boolean, default false), one additive step at execute:wave:post
  (onError: skip, gates: []), so it can never halt a wave
- agents/gsd-dom-verifier.md — the only GSD agent carrying browser MCP
  globs, in its own tools: line, with no Bash
- verify-work automated_ui_verification — a gsd:live-dom-families block
  naming both new families AND the key; presence alone never activates

Two independent fail-closed gates: isCapabilityActive renders a hook
only on state.active === true, plus the step's own `when`.

The pre-existing mcp__playwright__* branch keeps the gating it already
had and stays outside the new block. Pulling it behind a default-off key
would have silently removed working automated UI verification from every
current Playwright-MCP user on upgrade.

Also closes a host gap this surfaced: execute:wave:post dispatched only
contribution + gate, so ANY registered step was declared and silently
never run — exactly the single-kind hand-roll loop-hook-dispatch.md
names. Step 5.75 now dispatches every kind == "step".

The browser-profile lock is tolerated, not coordinated: --isolated is a
flag on the operator's own MCP-server registration that GSD neither
launches nor parameterizes, so the verifier reports could_not_look /
profile_locked, names the flag, and stops. DOM-VERIFY.md keeps
could_not_look and nothing_to_report distinct behind a closed reason
enum — collapsing them is the ambiguous-run-notes defect reported.

Verified on the remote runner.

Closes #2856

* fix(#2856): apply review findings from the orthogonal passes

Correctness pass (blocker):
- delete detectionBlockIsCrlfSafe. It was pass-always: it read the file,
  replaced LF with CRLF, then indexOf'd marker strings that contain no
  newline, so the replacement could not change the result and the
  assertion could never fail for the reason it stated. There is no real
  CRLF risk on this surface either — the gsd:live-dom-families block has
  no parser, only human and agent readers. Deleted rather than replaced,
  per the repo's pass-always-test rule.

Isolated security pass (two minors, both real):
- execute-phase.md step 5.75: this change is what first activates
  kind == "step" dispatch at execute:wave:post, which newly opens the
  ref.command shell path at that loop point. Our own step uses ref.agent
  and never touches it, but the door is now open, so the step-dispatch
  line carries the same in-context validate-before-shell warning the
  sibling gate-dispatch line directly below it already carries.
- gsd-dom-verifier: quoted page text in DOM-VERIFY.md is attacker
  influenced. Require it wrapped in inline code or a fence, kept short,
  and never left reading as a directive to the next reader.

Verified on the remote runner.

* fix(#2856): settle the new-agent roster ripple

Checkpoint 2 returned 28 failures, none in the new suite — all of them
the guards that exist to make adding an agent a deliberate act. Each is
a real boundary that had to move:

- docs/AGENTS.md: Tools row must copy the frontmatter verbatim (#2526),
  so the browser globs lose their backticks; primary-agent counts 21->22,
  roster 33/34->34/35, Verifiers category 1->2
- docs/INVENTORY.md: roster completeness requires every agents/gsd-*.md
  to be classified exactly once
- gsd-dom-verifier: add the anti-heredoc instruction and the commented
  hooks: frontmatter pattern both agent gates require
- gsd-core/bin/shared/model-catalog.json: every shipped agent needs a
  profile entry (#3229)
- copilot-install / kilo-upgrades / qwen-upgrades: expected agent list
  and the 34->35 roster boundary
- execute-wave-post-gate-pipeline-e2e: execute:wave:post legitimately
  carries one step now. Asserted as an exact shape — one step, capId
  live-dom-uat, ref.agent gsd-dom-verifier, onError skip — so it stays a
  real guard against accidental change rather than being relaxed

Two findings worth naming:

mcp-tool-inheritance (#2526) rejected the agent for documenting
mcp__playwright__* while its tools: line withholds it — a dead
instruction that invites the agent to claim a path it cannot take. The
prose now names the Playwright MCP family without the dispatchable
token, in both the agent and the capability fragment.

runtime-launcher-parity rejected the new gsd_run call: each fenced block
is its own shell, so a workflow step file invoking gsd_run needs its own
canonical preamble. Propagated with scripts/sync-runtime-launcher.cjs.
That script also normalizes explore.md, which is unrelated pre-existing
drift the parity check tolerates, so it is reverted to keep this diff
scoped.

The emitted-drift ack supersedes the spent #3370 entry for
execute-phase.md — it is merged into next, so its ripple is absorbed at
the base and it can no longer clear anything. That is the same supersede
the #3370 entry itself performed on the spent #3324 fragment. Its
unrelated execute-plan.md entry is untouched.

Verified on the remote runner.

* fix(#2856): drop the stale emitted-drift ack entry

The automated-ui-verification.md entry was written speculatively rather
than from a reported growth, and the check names that precisely: an ack
"written or reworded in THIS diff, but nothing here needed it, so it
explains nothing".

The growth tier keys on the bare filename as it appears under
gsd-core/workflows/ or agents/. automated-ui-verification.md is nested
under verify-work/steps/, so it was never in the tracked set — only
execute-phase.md was ever reported, both before and after the launcher
preamble landed.

Only ack what the check actually reports.

Verified on the remote runner.

8da2dd3ad2 feat(#2790): add read-only planning.inspect schema-v1 snapshot query (#3708)
* feat(#2790): add read-only planning.inspect schema-v1 snapshot query

Adds a read-only query emitting a schema-versioned JSON projection of .planning/
so downstream harness UIs can consume planning state without parsing GSD's
Markdown a second time.

Composed strictly from the ADR-3180 section 7 owners plus parsePlanDocument,
parseRequirements and parseUatItems; markdown structure is read through the
Markdown Sectionizer and Markdown Table Model seams. It declares its own flat
external schema rather than serializing PlanningSnapshot, which is the
diagnostic-rule subject and still growing.

Extracts plan-document parsing out of cmdPhasePlanIndex into a shared leaf
module so phase.plan-index and planning.inspect cannot drift, including the
plan-id derivation both surfaces report.

Also fixes parseRequirements dropping the separator delimiter used by the
shipped requirements template, surfaced while wiring the requirement rows.

* fix(#2790): close spec gaps and a raw-text test assertion found in review

Review findings from the standards, spec and security passes:

- phases[] rows carry goal and dependencies, the two per-phase elements the
  issue Summary names that had no corresponding field. Goal is bounded to the
  section's leading prose so the Depends-on line, the Plans checklist and the
  wave annotations are not duplicated into it.
- requirement rows carry their own diagnostic codes, so a consumer no longer
  has to string-parse the global diagnostics subject to correlate.
- roadmap_acceptance.checkbox is looked up through the phase-id key owners.
  It was compared raw against the on-disk directory name, so it read null for
  every real-world slugged phase directory and the evidence channel was inert.
- the hostile-input test asserts the structured payload instead of matching the
  raw stdout string. The absence proof over raw stdout is kept deliberately.

* fix(#2790): register planning in the runtime usage list and repair fixtures

Remote runner reported 9 failures on 9b3f9aa. Two root causes, both fixed:

- gsd-tools.cjs registered the planning family in HOST_COMMAND_ROUTERS but
  never added it to TOP_LEVEL_USAGE's Commands list. Those are two surfaces a
  parity test guards, and the top-of-file block comment is not the runtime
  help string. A real wiring gap that every local gate and three review passes
  missed.

- the new suite's fixtures could not produce a resolvable phase set. STATE.md
  frontmatter omitted the milestone field, which ADR-3180 7.2 rule 1 makes the
  primary milestone selector, so the phase set scoped unscoped and every
  percentage was correctly withheld. Separately declarePhase returned a path
  without creating the directory, so a phase declared but never written to left
  phases empty. Both reproduced against the built module before fixing.

No assertion was weakened. The withholding path is still exercised and still
returns null when the roadmap is absent.

* chore(#2790): backfill changeset pr number

* test(#2790): cover every enumerated matrix row and contain a symlink escape

Reverses a silent deferral. An earlier revision left 23 of the 78 enumerated
matrix rows unimplemented and 7 more as one-off manual checks, with a paragraph
in the artifact and the PR body describing the gap. CLAUDE.md is explicit that
such a note is not a fix and is not surfacing. The rows are implemented instead
and the manual-evidence bucket is gone: 49 test cases become 88, covering all 78.

Writing the symlink row proved a real leak: a *-PLAN.md symlinked outside
.planning/ had its content emitted into the payload, confirmed via a direct call
and the spawned CLI. readDocument now resolves target and planning root with
realpathSync and rejects an escape, returning the ordinary unreadable-document
shape. Tested both ways, because a containment check that over-rejects is its own
defect: an escaping symlink leaks nothing and degrades that plan alone, while a
legitimately relocated .planning/ symlink stays fully readable.

The three new modules are registered in the mutation COVERED registry, which had
been reporting has_work false and skipping the Stryker gate entirely. Provisional
non-binding floors so the shards run and report; raised to the measured value
before merge, since the registry forbids calibrating from a local run.

* fix(#2790): satisfy the mutation ratchet contract and scope the 1MB test

Remote runner reported 16 failures on 8c451ed. Two causes.

The COVERED registry has a paired contract the earlier commit violated: every
module needs a matching RATCHET_BASELINE entry, and minScore must be between 50
and 100 with minScore === baseline. The provisional floor of 1 was illegal on
both counts. All three modules now sit at 50 — the registry's own enforced
minimum — with matching baselines. The score cannot be measured locally: the
shard runs node --test, which this repo hard-blocks, so CI is the only source.
Floors are raised to the measured value once this PR's shards report; a shard
below 50 means the tests need strengthening, since the floor cannot go lower.

The 1MB test was measuring the test harness rather than the product. The command
handles the oversized payload correctly by spilling to a tmpfile and resolving it
back, but the resolved stdout then exceeds runGsdTools' maxBuffer and the helper
reports ENOBUFS. It now uses --pick so stdout stays one byte while the full 1MB
document is still read and parsed end to end.

* fix(#2790): wire containment across every document read this command drives

An isolated security review of the containment control found the boundary logic
sound but not comprehensively wired: two content reads reached the filesystem
without it.

An escaped phase DIRECTORY could enumerate external filenames into the file
fields and diagnostic subjects. Both enumeration sites now containment-check the
directory before reading. Worth recording that the leak was already prevented one
layer earlier than the review claimed: Dirent#isDirectory() reports false for a
directory symlink, so such a directory never becomes a phase row at all. The
guard is defense-in-depth for a direct caller and for platforms where a reparse
point reports as a directory.

A *-VERIFICATION.md symlinked outside the root leaked one frontmatter value
verbatim, because readVerificationStatus does its own read and copies an
unrecognized status into the payload's next_action. Closed from the consumer
side through that function's existing fs injection seam, so src/verification.cts
keeps its signature and its other callers are untouched.

The reviewer additionally rated a forged status: passed as an integrity bypass.
It is not: anyone able to plant the symlink can plant a real VERIFICATION.md
saying the same thing. The incremental risk is confidentiality, which is what
these fixes close.

src/plan-scan.cts is deliberately unchanged: isPlanSuperseded reads
symlink-followed content but yields only a derived boolean, no document text.

* test(#2790): give the mutation shards an in-process surface

Two Stryker shards were CANCELLED at the 15-minute cap, not failed on score.
CI log: 640 mutants instrumented, and the dry run reported 'Ran 1 tests in 20
seconds' because the shards pointed at the integration suite, where nearly every
case spawns a gsd-tools subprocess and Stryker's command runner treats the whole
test-runner invocation as a single test. 640 x 20s cannot finish in 15 minutes;
at the kill it was 27/640 with an ETA over an hour.

Every other COVERED module points at a property or unit file, and the workflow's
own paths filter lists exactly those two patterns. In-process is the intended
mutation surface; the shards were pointed at the wrong shape of test.

Adds tests/planning-inspect.unit.test.cjs — 39 cases in 10 describes that spawn
nothing and call the built modules directly. plan-document and the router need no
filesystem at all, one being a pure content-to-object parser and the other taking
an injected mock. The three shards now point here. The 91-case integration suite
is untouched and still runs in the normal test job.

* chore(#2790): ratchet mutation floors to the measured CI scores

CI run 32392791843 measured all three shards, which is the only source the
registry accepts — local runs count timeouts as kills and inflate badly.

  planning-command-router  95.65 -> floor 94
  plan-document            76.58 -> floor 75
  planning-inspect         57.03 -> floor 56

Applied the registry's own rule, floor(score) - 1, and updated RATCHET_BASELINE
to match, since the ratchet test enforces equality.

planning-inspect sits well below the file's target of 80 and is the obvious
ratchet candidate as its tests improve. planning-command-router already exceeds
the target. The placeholder comment about floors pending measurement is removed
rather than left standing as a false statement.

77fa08f1e8 fix(#2773): feed the spec-phase edge probe English-translated requirement text (#3713)
* test(#2773): failing-first contract and premise tests for translated edge-probe input

Locks the Step 5.5 contract that a response_language project must feed the
edge probe an English translation of each requirement's text, and binds that
advice to measured engine behavior: the same requirement classifies to zero
shapes in Portuguese and to collection/adjacency/empty/ordering in English.

Also pins the honest limit — the issue's own repro sentence classifies to []
in English too, so translation is necessary but not sufficient and the
authored shapes override is the documented fallback.

Red before the doc change; the assertions are all false today.

* fix(#2773): feed the spec-phase edge probe English-translated requirement text

The shape cues in src/edge-probe.cts are English word-boundary regexes, so a
project running with response_language set wrote its SPEC requirements into the
Step 5.5 $REQS_JSON heredoc in that language, matched no cue, classified to zero
shapes, and landed every row in the unclassified sentinel (#1110). The taxonomy
contributed nothing and --auto left it all unresolved — the probe was a silent
no-op for exactly the spec type it exists to harden.

Step 5.5 now states that the $REQS_JSON payload is engine input rather than
user-facing output, so the response_language rule does not govern it: each
requirement's text carries a faithful English translation, the SPEC keeps its
original language, and requirement ids are never translated or renumbered. The
instruction sits before the heredoc on purpose — the downstream APPLICABLE=0
warning fires only when every requirement is unclassified, so a partly-classified
non-English spec would otherwise slip through with no signal at all.

Measured against the compiled engine: the same requirement returns [] in
Portuguese and collection -> adjacency/empty/ordering in English. Also measured:
the issue's own repro sentence returns [] in English too, so translation is
necessary but not sufficient — the instruction therefore points at the authored
shapes override for prose carrying no cue in any language rather than promising
that translation restores classification.

Doc scope only, per the triage disposition on the issue. The compiled engine is
untouched; the lang-hint / per-language cue-set fix is a separate follow-up.

* fix(#2773): clean up the edge-probe temp file on the placeholder-guard exit path

Surfaced by the isolated security review of this branch. Between the mktemp and
the unconditional cleanup, Step 5.5 has two sibling guards that disagreed about
their own invariant: the engine-failure guard runs rm -f "$REQS_JSON" before
exiting, while the empty/placeholder guard directly above it exited without one.
A spec run that tripped the placeholder check therefore stranded a temp file
holding the SPEC's requirement text in TMPDIR, once per failed run.

The added contract test walks the region between the mktemp and the
unconditional cleanup and asserts no exit path leaves the file behind, so the
two guards can no longer drift apart. Proven to bind: run against the pre-fix
file the walker reports the leaking exit; against the fixed file it reports none.

reads when it turns the setting on; it now names the one deliberate exception

CONTEXT-INDEX.json regenerated via gen-context-index.cjs --write. The drift-ack
fragment is updated for the final byte range and now also records the
placeholder-guard cleanup fix folded into the same block.

The remote runner caught this: emitted-attribution.test.cjs pins the
0000-legacy-migration.json spec-phase.md entry permanently (the #2914 migration

removing it to avoid a duplicate-key collision with a new fragment broke that
test instead of satisfying the ratchet.

The entry is an accreting log, not a single-use slot — #2733, #3132 and #3102
were each appended to the same reason string by later PRs, which is how a shared
growth key coexists with the rule that two ack sources may never name the same
path. This appends the #2773 rationale the same way and drops the separate
fragment, whose spec-phase.md key was the collision.

Verified locally by reproducing both affected tests against the real fragment
before re-dispatching: the pinned delta survives, grown[0].acked is true,
staleAcks is empty, and all 35 entries still read as spent.

The phase gate's enablementSequence check caught a wrong call of mine. I had
recorded that no how-to was owed because the user takes zero extra steps — the
workflow translates the probe input itself. Written out, though, the sequence
from off to value is two steps and step 1 depends on response_language, a
setting owned by a different capability than the edge probe, which is exactly
the condition the how-to test names.

There is also real task content a reference table cannot carry: the three-way
split between a few unclassified rows (the classifier's recall gap), every row
unclassified (the probe could not read the spec at all), and the silent
partly-classified case where the APPLICABLE=0 warning never fires. That last
one is what a user would otherwise misread as a clean bill of health.

Shaped after the resolve-edge-coverage-findings / resolve-unreachable-guard
siblings and indexed from docs/README.md next to its closest relative.

Adds an opt-in `state ~N commits back` marker to the GSD-state segment,
consuming the `state_head` stamp and freshness contract landed by #2622.
A solo developer returning to a project reads "Phase 4, executing" in
STATE.md and acts on it, without noticing the codebase moved 40 commits
since that line was written. /gsd-health reports it as W024, but only if
you think to run it; the statusline is the surface you see without asking.

Fires at STATE_HEAD_ADVISORY_COMMITS (20), the same threshold W024 uses,
not at > 0: with commit_docs:true the commit carrying a STATE.md sync
advances HEAD by one, so > 0 would alarm permanently on a fresh project.

Costs exactly one bounded git subprocess per render and none when
disabled. `rev-list --left-right --count` answers ancestry and distance
together, and repo pinning is a filesystem check mirroring
projectOwnsItsRepo rather than a --show-toplevel compare, which is
unreliable on macOS /private/var and Windows 8.3 paths.

Every unresolvable input degrades to the tri-state unknown -- the marker
is absent, never a "fresh" claim the project cannot substantiate: a
malformed stamp, a root that does not own its .git, a sub_repos
workspace, history rewound past the stamp, or git being unavailable.

Also collapses statusline config resolution onto one resolveStatuslineOptions()
seam. runStatusline() and renderStatusline() duplicated it byte-for-byte;
one copy is what keeps a newly-added key from reaching only one of them.

* test(#2734): route the e2e spawn through the process seam and fix fixture leaks

Review findings from the two orthogonal passes:

- `bothEntryPointsResolveOptionsIdentically` spawned a child and substring-matched
  its stdout to test a pure function. It now calls resolveStatuslineOptions()
  directly — no subprocess, no text matching.
- `skipsFreshnessWorkWhenTodoTaskActive` genuinely needs a child (the !task gate
  lives in runStatusline, which reads stdin), so it now spawns through
  tests/helpers/process-seam.cjs and proves the negative with a filesystem fact:
  the git shim appends to a marker file on every invocation, and the assertion is
  that the marker never appears. Stronger than asserting text is missing, and it
  drops the last stdout substring match in the block.
- Every fixture-creating test now registers `t.after(() => cleanup(dir))` instead
  of a trailing cleanup(dir), which leaked the temp repo on assertion failure.
  derivationAgreesWithStateModule reassigns `dir` across five fixtures, so it
  binds each directory at scheduling time rather than cleaning only the last.

Also corrects markerCoexistsWithMilestoneComplete, which asserted the wrong
expectation rather than finding a code defect: `percent` drives the progress bar
too, so the milestone segment reads "v1.9 [##########] 100%". The marker appends
after it, which is what the test exists to prove.

CONTEXT.md's opt-in statusline key list was missing statusline.show_git as well
as the new key; both are now enumerated.

Adds workflow.code_review_depth_overrides — an ordered array of {paths, depth}
rules matched against a review's changed-file set by segment-aware path-prefix
comparison. Resolution order is --depth= flag, then the strongest matching rule,
then workflow.code_review_depth, then standard; a matching rule replaces the
global rather than being max'd with it, so quick and standard rules stay
meaningful. Glob metacharacters are a hard configuration error rather than sugar
for a prefix, and malformed rules halt the review instead of degrading to
standard. The resolver is pure and reports its own provenance, so the workflow
can print the resolved depth and the rule that matched. The pre-existing
>50-file deep-to-standard downgrade moves into the module and now names the rule
it overrode.

The key is registered centrally rather than as a capability config slice: the
federated slice channel admits only boolean/string/number/enum, so an array
slice would be dropped as malformed.

* test(#2554): correct depth-provenance assertions and pin out-of-repo paths

Two corrections to the failing-first suite. The source assertion for a
non-matching rule with no global configured expected 'config'; with no global
set the depth comes from the default, and a companion assertion tolerated
either value, so both passed against an implementation that derived provenance
from whether any rules existed rather than from where the depth came from.

The out-of-repo absolute-path case used a home-directory path that matched
neither implementation, so it never exercised the defect it named. It now pins
the discriminating cases: an absolute path outside the repo root must not match
a repo-relative rule, and one under the root must.

* docs(#2554): document path-scoped code review depth overrides

Reference rows for workflow.code_review_depth_overrides in the configuration,
features and commands references plus the locale copies that carry those tables,
and in the planning-config reference. Explanation of why escalation is
whole-review rather than per-file and why v1 is prefix-only. New how-to for
scoping review depth by path, carrying the configuration-error reason table and
the distinction between nothing to report and could not look. CONTEXT.md
glossary entry and the INVENTORY row for the new CLI module.

ja-JP and ko-KR CONFIGURATION.md carry no code_review keys at all, and ko-KR and
pt-BR FEATURES.md carry no code-review config table, so those files are
deliberately untouched.

* fix(#2554): make the depth-misconfiguration halt executable and reject control chars

Three review findings, all in this change.

The misconfiguration halt was prose rather than shell: the error-printing fence
was followed by an unconditional extraction fence, so an ok:false result threw
and left the depth empty instead of stopping the review. Prose is not a guard —
the two fences are now one block with a real conditional, and anything that is
not the literal string true fails closed.

An interior control character in a rule path survived validation and reached the
provenance string and the summary box; rule paths now reject control characters
via a new PATH_CONTROL_CHAR reason, after the glob check so precedence is
unchanged. That in turn makes the field record safe to delimit, so the seven
node invocations that each re-parsed the same result to read one field collapse
to one.

Also corrects the glossary entry's illustrative paths, which the glossary-ref
check read as real repository references.

* fix(#2554): use the fast-check v4 string API and acknowledge workflow growth

Two failures from the remote matrix on d3111f45, both this branch's.

The property block built its segment arbitrary with fc.stringOf, removed in
fast-check v4. Because the arbitrary is constructed in the describe body, the
throw took out all four property tests rather than one — they had never
executed. Rewritten to fc.string({unit, ...}), the form this repo already uses
in emitted-attribution.test.cjs. Every other fast-check helper in the file was
audited against the installed module.

The emitted-attribution growth arm needed an acknowledgment for code-review.md,
which grew 5376 bytes. The pre-existing 3503 fragment keying the same file is
spent — its ripple was absorbed when #3503 merged, and the base file is exactly
the 34435-byte baseline this growth is measured against — so it cannot clear
anything, while the ack lint hard-fails on a duplicate key across two sources.
Removed it in favor of the new fragment, which is exactly how #3503 itself
replaced the spent 3191 fragment.

- gsd-plan-checker: new Verify Command Path Resolvability dimension that
  reports the failing target and never prescribes a replacement.

Also fixes first-match-wins prefix bucketing in scripts/lint-test-file-count.cjs
(readdir order is not stable across platforms, so a module whose name extends
another's with a hyphen bucketed differently on Linux than on macOS).

- --prefix captured with \S+, so a quoted path containing a space was

  is now quote-aware.
- A chained cd whose later segment was absolute concatenated instead of
  resetting, producing a nonsense path and another false blocker. The

phase-id helpers. Real phase directories are NN-slug, not phase-N-slug,
so the prior-command harvest matched nothing outside its own fixtures
and the planner-inheritance half of this feature was dead code.

cannot see it. The dynamic-path refusal rejected ~ anywhere, and a
GitHub Windows runner's tmpdir is an 8.3 short name -
C:\Users\RUNNER~1\AppData\Local\Temp - so a valid absolute Windows
path came back unresolvable/dynamic_path.

This was a production bug, not a test artifact: any Windows user whose
project path carries an 8.3 short name, or any literal ~, silently lost
the probe entirely - every command degrading to unresolvable with no
explanation.

~ is a home expansion only at the start of a path; elsewhere it is an
ordinary literal. The check is now split: $, backtick, *, ? and newline
stay refused anywhere (substitution and globs, and the glob characters
are illegal in Windows path components regardless), while ~ is refused
only leading, tolerating one leading quote since the check runs before
quote stripping.

The prior tests only caught this on Windows because only Windows puts a
~ in tmpdir. Four new tests pin it on every platform via a fixture
directory literally named RUNNER~1.

Review BLOCKER. smart-entry.cts::detectVerifyFailed resolved the current
phase's directory with its own `.find(phaseTokenMatches)` and never
reached the shared selection, so the bare-integer-fallback family the
issue names — `05-80-20-cleanup`, `30-12-factor-refactor` — resolved
nowhere. The miss is silent by construction: an unresolved phase reports
"not failed", which is byte-identical to a healthy one, so a failed
verification simply never surfaced in /gsd or /gsd:progress.

`entries` is already sorted and matchPhaseDirs filters without
reordering, so matches[0] reproduces the previous selection exactly
wherever the old code resolved at all.

Wiring it into phase-resolution-parity.test.cjs as a fourth path then
exposed a second, older defect in the same function: phaseTokenFromDirName
shape-probed the UNSTRIPPED token, so a project-code-prefixed directory
(`MEM-05-…`, tokenizing to `MEM-05-80-20`) failed the leading-digit test
and was dropped before any resolution ran — every phase in a
project-coded plan was invisible to this check. The probe now runs on the
stripped token; the returned value is unchanged, so the comparePhaseNum
sort is untouched.

Path 4 has no JSON surface to compare, so the gate observes selection
indirectly: plant the failing artifact in exactly one directory and a
passing one everywhere else, then read the boolean. Reverting either fix
turns 5 of the 10 corpus scenarios red.

* refactor(#2528): collapse the duplicated extractCanonicalPlanId

Review MAJOR. The function existed as two independent, byte-identical
copies — src/core-utils.cts and src/phase.cts — and this PR had to patch
BOTH with the same single-digit-slug rewind rule. That is the generative
fix divergence CLAUDE.md names, and only the core-utils copy was under
test, so a future one-sided patch would have silently split plan-id
canonicalization between the plan listing and everything else.

Removed rather than parity-tested: core-utils was already the leaf owner
and already exported it, and phase.cts already imported that module, so
there is no second surface left for a parity test to police.

* test(#2528): pin matchPhaseDirs at the digit-width boundaries

Review MAJOR. The bare-integer fallback's correctness rests entirely on
capturing each directory's whole leading digit run before the zero-strip
compare; a regex that stopped short would turn every query into a prefix
match, and "1" would claim 10, 100, and 12 alike. The existing coverage
was example-based and never touched that boundary.

Adds the explicit 9/10 and 1/10/100 cases — including the forms where
only the wider directories exist, so an exact-width neighbour cannot
satisfy the assertion — plus a fast-check property over arbitrary
distinct leading runs. The property is stated as an invariant on the
result (every returned directory's leading run IS the query) rather than
an expected list, so it covers primary and fallback matches alike and
cannot be satisfied by reimplementing the selection in the test.

Both fail when the fallback regex is degraded to a prefix match.

* fix(#2528): route the remaining eight consumers through matchPhaseDirs

phaseTokenMatches had eight consumers left that each rebuilt the directory
selection around it by hand: phases-list, next-decimal, phase-remove, the
W021 milestone-consistency check, schema-drift, the init-manager overview,
milestone-complete's disk check, and roadmap analyze. Every one of them
reproduced the reported symptom in full after the tokenizer was fixed.

None of them derives a displayed phase number from the matched directory,
so none needs phaseNumberForMatch; the change at each site is the
selection and nothing else. matchPhaseDirs filters without reordering, so
matches[0] reproduces the prior .find() choice wherever the old code
resolved at all.

exported as the primitive matchPhaseDirs is built from and as a pinned
canonical surface, but no consumer reaches past the owner to it.

* test(#2528): extend the parity gate to the migrated consumers

Each of the eight is observed through the surface a user sees, not
through the matcher, with a no-directory control so the assertions cannot
be satisfied by a consumer that resolves unconditionally. init-manager
and roadmap-analyze are additionally asserted to agree with each other.

* refactor(#2528): own the case-flexible phase grammar and the leading-digit-run fragment

validate.cts derived its case-flexible regex sources by running
`replaceAll('A-Z', 'A-Za-z')` over two constants exported by phase-id.cts.
That passes lint-phase-id-drift.cjs — there is no literal copy of the
grammar — but it depends on the owner rendering that exact substring. The
day phase-id.cts expresses the same class any other way the replaceAll
silently no-ops and validate.cts narrows to uppercase-only. The failure
mode is a NON-match, so nothing throws and no uppercase-only fixture
notices. Both variants are now derived once, beside the sources they
widen, and imported.

Also names the leading digit run the bare-integer fallback selects on.
It was spelled `/^(\d+)(?:-|$)/` where the fallback filters and `/^\d+/`
where phaseNumberForMatch reads the number back off the winner; selecting
on one run and displaying another would resolve a directory and then label
it with a number that never matched it.

* fix(#2528): refuse to remove a phase when two directories claim its number

cmdPhaseRemove was the only migrated site taking matches[0] with no
multi-match guard. Every sibling resolution path returns ambiguous_matches
and refuses to choose; this one is the DESTRUCTIVE path, so choosing
silently is strictly worse than anywhere else. With 05-80-20-a and
05-90-till-late on disk, `phase remove 5 --force` deleted one of them and
renumbered every phase after it — where the base resolved nothing, deleted
nothing, and the corpus in tests/phase-resolution-parity.test.cjs already
declared that exact input ambiguous.

The refusal is emitted before any file is touched and carries both
candidates. CONSUMER_SCENARIOS could not express the case — every row is
binary, resolving to one directory or to none — so the gate gains a
dedicated ambiguous test. It asserts on the filesystem, not only on the
reported directory_deleted: a null printed after an rmSync would satisfy
every other check.

* fix(#2528): pair digit-leading phase directories with their roadmap phase in validate health

W006/W007 are the ninth site of this bug class and the one a
`phaseTokenMatches` grep could never surface: they resolve roadmap↔disk by
intersecting TOKEN SETS, which is a dir→token labelling rather than the
query→dir selection matchPhaseDirs owns. On the canonical fixture the
label is wrong in both directions at once, so `validate health` reported
"Phase 5 in ROADMAP.md but no directory on disk" AND "Phase 05-80-20
exists on disk but not in ROADMAP.md" for the same directory.

collectDiskPhases now keeps the directory names behind each token, so
W006 can ask the canonical matcher whether a roadmap phase resolves to a
real directory, and W007 — which iterates directories and therefore has no
query to resolve — gets the inverse mapping it never had: a directory is
claimed when some roadmap phase resolves to it.

Both checks are additive: the token intersection still decides every shape
it already decided, and the resolution can only REMOVE a warning. The
regression test carries controls in the other direction — a roadmap phase
with no directory must still raise W006, an unclaimed directory must still
raise W007 — so it cannot be satisfied by a check that stopped reporting.

* docs(#2528): state and pin the directory-side scope of the bare-integer fallback

The matchPhaseDirs docblock claimed deep-decomposition lookups were
untouched. That is true of the QUERY side only — no non-bare query enters
the fallback — but the DIRECTORY side is what changed classification: a
bare `5` now reaches a lone `05-01-auth` and resolves it (phase_number
"05", phase_name "01-auth") where the base found nothing.

The widening is irreducible from directory names alone. `05-01-auth`
(sub-phase 5.1) and `30-12-factor-refactor` (phase 30 named "12-Factor
Refactor") are the same `NN-NN-<slug>` shape, and the discriminator that
would separate them — "is the second segment a valid decimal sub-phase" —
accepts `5.1` and `30.12` equally. Any rule strong enough to exclude the
first excludes the second, which is the defect #2528 exists to fix. So the
tie is broken in favour of resolving, the docblock now says so, and the
consequence is bounded where it matters: two such directories are two
matches, and every caller (including phase remove) refuses to choose.

Pins both directions, since nothing observed the directory side before.

* fix(#2528): count surviving phases by identity in phase remove's STATE resync

#2640 landed on `next` while this branch was open. Its STATE.md phase-count
resync re-derives "which directory was removed" from the query with
`phaseTokenMatches`, which is the tenth site of this issue's defect: the
bare-integer fallback resolves `05-80-20-cleanup` for query `5`, but the
token predicate does not, so the just-deleted directory is counted as still
present and the written `Total Phases` is one too high.

`targetDir` already IS the directory that was removed, and the block is gated
on it being non-null, so identity answers the question exactly — which is also
what the comment above the filter already claimed it did. This keeps
`phaseTokenMatches` out of `phase.cts` rather than re-importing it to satisfy
one call site: the module's public surface should not grow for a question that
does not need re-derivation.

Pinned in the parity gate with a control on a directory the tokenizer reads
correctly, so the assertion is about the digit-leading shape and not about the
counting rule changing for everything.

* fix(#2528): let the resolution layer own the digit-leading slug family alone

The tokenizer rewind this fix carried — pop the last absorbed continuation when
the segment that stopped the scan is a bare single digit — reads
"10-24-7-autonomy" (phase 10 named "24/7 Autonomy") correctly and silently
re-reads "10-24-7-zip" (sub-phase 10.24 named "7-Zip Integration") from "10-24"
to "10". The two names are string-identical in shape, so no local signal
separates them; the rule traded the reported ambiguity for the symmetric one a
level down, on a 15-caller chokepoint whose output also feeds query-less
derivations (STATE.md phase counts, W007, the #2562 key surface). A well-formed
sub-phase directory became unresolvable by its own id — the very symptom #2528
was filed about.

It also bought nothing. The bare-integer fallback in matchPhaseDirs already
resolves "10-24-7-autonomy" for query "10" whatever the token is: no primary
match, bare query, leading digit run "10". The reported case was covered twice,
by two rules, and the two disagreed about the case nobody reported.

So the rewind is removed rather than narrowed, in the tokenizer and in the five
surfaces kept in lockstep with it (BRACKET_PHASE_TOKEN_SOURCE,
PHASE_TOKEN_FROM_DIR_RE, canonicalPlanStem's pair grammar and its collision
branch, roadmap-parser's numericRe, extractCanonicalPlanId), together with the
SINGLE_DIGIT_RUN_SEGMENT_SOURCE owner constant they shared. Disambiguation now
lives only where a QUERY exists to disambiguate against, which is the same
bounded mechanism the "05-80-20-cleanup" shape already used.

Measured, not argued: over 29800 generated directory names, extractPhaseToken is
byte-identical to `next` on every input except the lowercase-continuation class
("01-20a", "05-80-20-25abc") — a rule about the segment itself, not a guess about
its neighbour.

Both readings now stay reachable by their own ids:
  matchPhaseDirs(['10-24-7-autonomy'], '10')    -> the dir   (fallback)
  matchPhaseDirs(['10-24-7-zip'],      '10')    -> the dir   (fallback)
  matchPhaseDirs(['10-24-7-zip'],      '10-24') -> the dir   (primary)

* test(#2528): pin the one-continuation boundary the rewind had no coverage for

The regressing shape was invisible to the suite by construction, not by luck:
the deep-rewind property built its cases from `continuationArb` with
`minLength: 2`, so it never exercised the single-continuation case — exactly one
genuine sub-phase level before a digit-leading slug — and every hand-written
fixture used the ambiguous shape only where "phase-plus-slug" was the intended
reading.

`continuationArb` is now `minLength: 1` and the property states the invariant
instead of the old rule: for any prefix, phase, 1-5 continuations and any
one-digit terminator, the token equals the FULL continuation run on both the
imperative and the regex surface, and `matchPhaseDirs([dir], token)` returns that
dir. That third assertion is the one that catches the class on its own — the old
behaviour made a well-formed directory unresolvable by its own id, which is a
property, not a fixture.

Around it: "10-24-7-zip" and "10-24-3d-printer" now sit beside
"10-24-7-autonomy" everywhere the family is pinned, so the two readings can never
diverge again; the 24/7 metamorphic property asserts the RESOLUTION result rather
than the token (the token is precisely the part no surface may decide); the
end-to-end parity corpus gains "a sub-phase with a digit-leading slug resolves by
its full id" across all four resolution paths; and the milestone-scoping residual
is pinned in three directions rather than left to prose.

Mutation: re-inserting the rewind and rebuilding turns 9 tests red, the
`minLength: 1` property first, and nothing else. Build success checked separately
(build:lib reports 0 `error TS`), so the mutation reached the artifact under test.

* test(#2528): pin the #2946 guard against digit-leading phase directories

The #2946 fix makes the milestone-complete unstarted-phase guard run
unconditionally, so whether it fires now rides entirely on the
directory-resolution owner this PR replaces. Two cases, both with STATE.md
carrying no `milestone:` field so the #2946 path is the one exercised:

  - ROADMAP Phase 5, disk `05-80-20-cleanup` → guard must stay silent.
    RED on next (fail-closed: the guard blocks a legitimate one-way-door
    operation because phaseTokenMatches resolves neither 05 nor 80 for
    that directory).
  - ROADMAP Phase 80, same directory → guard must still fire. Green on
    both sides; it pins the fail-open direction against a future widening
    of the matcher.

* fix(#3175): stop the injection scanner reading RegExp.exec as code execution

The apostrophe fix in 27aa40f6 replaced ["\x27] with a real ["'] class.
The old class never contained an apostrophe at all (POSIX bracket
expressions do not honour backslash escapes, so it was the set ", \, x,
2, 7), so only exec(" matched. Single-quoted method calls now match for
the first time, and RegExp.prototype.exec takes a subject string, not
code: any PR touching a file that tests a regex goes red. On next, six
files match the scanner's own pattern across 16 method calls.

A plain [^[:alnum:]] boundary cannot separate the two forms because . is
not alnum, so exec gets [^[:alnum:].] and the command-execution vector
moves to a dedicated member-call pattern. Bare exec('rm -rf /'),
cp.exec(...) and child_process.exec(...) all still fire.

Mutation: reverting the boundary reds 1 test and only it; removing the
member-call pattern reds the 2 non-weakening tests and only them.

The left boundary [^[:alnum:].] added in 58a7b560 excluded a preceding dot,
which dropped every member-position .exec('…') from the scanner. The follow-up
receiver pattern only restored three literal spellings (child_process,
childProcess, cp), so require('child_process').exec('…') — the most common Node
spelling of the vector this pattern exists to catch — became invisible, along
with any opaque receiver (conn.exec, shelljs.exec).

Revert the pattern to its receiver-blind form and handle the RegExp.prototype
.exec false positive where the script already handles this class: per-file
ALLOWLIST entries for the two phase-token grammar suites. Mutation-checked —
removing the two entries reds exactly those two files and nothing else.

The four assertions written around the old patterns are replaced by a
table-driven set covering all six spellings, including the three the narrowed
pattern silently lost.

- BRACKET_PHASE_TOKEN_SOURCE: the (?=-|$) terminator is what keeps the bracket
  read path in step with the other surfaces, and it costs the display shapes
  (`05.03: Title`, `12A: X`, `05.03]` no longer tokenize). Pinned so widening
  the terminator class is a deliberate act rather than a lookahead deletion.
- canonicalPlanStem: uppercase plan suffixes still strip, lowercase and dotted
  sub-plans now fall through. Dead export; pinned as a decision on record.
- getMilestonePhaseFilter: `12A-01-foo` now yields `12A-01`, matching what
  `12-01-foo` has always yielded. The letter suffix was the only reason a
  sub-phase directory folded into its parent phase's milestone window; the two
  shapes now agree. Not named in the review — found auditing the same commit.
- matchPhaseDirs docblock claimed every caller refuses on multi-match. Four do;
  five take matches[0]. Replaced the claim with the actual two-tier policy and
  the honest caveat that the bare fallback makes multi-match newly reachable

2dbee3ebdd enhance(#2229): add three-way claim disposition (admit/refute/abstain) to /gsd-explore research pass (#2543)
Closes #2229.

Each claim surfaced by /gsd-explore's research pass is dispositioned admit, refute, or abstain, with abstentions routed to a visible ledger instead of being smoothed into confident prose. Refute and abstain are separated by whether the disagreeing source is authoritative for that claim; a strong prior is never authoritative alone.

Two guards ride with it: conflict-abstention, and a tier floor that presents a would-be admit as an abstain when the researcher's resolved tier is the budget tier or cannot be determined.

To make that floor enforceable, resolve-model now emits the effective tier (--pick tier). It was already computed above the resolve_model_ids omit gate but was unreachable from a workflow, which left the floor inert on every non-Claude install - the model id is blank under omit and runtime-substituted where a tier map exists, and the profile defaults to balanced. The tier signal mirrors every resolution step that can change which tier runs, including the model_policy preset, and reports unknown rather than guessing. Output is additive; model, profile and effort are unchanged.

Two residuals are disclosed in the workflow rather than papered over: a raw-model-id model_overrides pin reports unknown and is floored (fails closed), and a model_profile_overrides entry repointing a tier at another tier's model can under-report (fails open, and predates this change).

Admin merge used only to satisfy the missing secondary reviewer on a single-maintainer PR. No CI failure and no conflict were bypassed: 38 checks green, remote runner 32255/32255 on both Node lanes.

* test(#1953): failing-first suite for the complexity-triggered refactor hook

60 behavioral cases against src/complexity-trigger.cts, which does not exist yet:
decision-point counting, the comment/literal stripping leak surface, threshold and
jump-delta boundaries at limit-1/limit/limit+1, stable-anchor baseline semantics,
and fs fault injection via mock.method. Two fast-check properties assert that
stripping never manufactures a decision point and that comments and string
literals are score-neutral.

Also registers the refactor-trigger capability manifest (inert until
refactor.trigger_enabled) and regenerates the capability registry and matrix.

Verified RED on the remote runner before any implementation exists.

* feat(#1953): complexity-triggered refactor extension point

Adds the opt-in refactor-trigger capability. After a phase executes, an
execute:post step measures per-function complexity for the files the phase
touched and writes a scoped refactor proposal when a function crosses the
configured threshold or drifts past its recorded anchor.

Design notes worth carrying:

- The signal is computed in-core (decision-point counting over comment- and
  literal-stripped source, Node builtins only) rather than via Memtrace or a
  shelled-out analyzer. The hook fires as a deterministic CLI, not an agent
  with MCP tools, and core takes no external dependencies — this is the only
  option a behavioral test can bind to. The metric sits behind a seam.
- The baseline is a stable anchor, not a rolling value: set on first
  observation, moved only on disposition. A rolling baseline makes the delta
  the single-phase change, so a function creeping +2 per phase never trips a
  delta of 5 and the jump check adds nothing over the absolute threshold.
- Strict mode records an open deviation window in the broken-windows ledger
  rather than declaring its own ship:pre gate. ship.md has no generic ship:pre
  gate dispatch — only two hardcoded branches — so a third gate of any kind
  would be declared and never evaluated.
- The gate clears on the proposal being dispositioned, never on the score
  improving. A blocking complexity number is one an executor can satisfy by
  splitting a coherent function in two.

execute-phase.md gains a generic execute:post step-dispatch contract; it
previously matched only ref.skill == "code-review", so any other step
registered there was declared and never run. The code-review branch is
unchanged.

Full rationale in ADR-1953.

* fix(#1953): close git option injection and symlink escape in the refactor hook

Three findings from the isolated security review, all fixed inline.

HIGH — changedFilesSince interpolated the --since value into a revision
token placed before the -- separator. A -- only stops PATHSPEC parsing of
arguments after it; git still option-parses what comes before. So
--since '--output=/tmp/x' became --output=/tmp/x..HEAD, which git accepts
as --output=<file> and uses to redirect diff output — an arbitrary write.
Fixed with --end-of-options before the revision range plus a conservative
ref validator. The validator deliberately permits ~ ^ @ { } because those
are legitimate git REVISION syntax (HEAD~1, main@{yesterday}) as distinct
from ref-NAME syntax; --end-of-options is the actual barrier. The doc
comment asserting the trailing -- was sufficient was wrong and is corrected.

MEDIUM — resolveConfinedPath confined by string prefix only, so a symlink
committed inside the repo passed the check (its own path is under cwd) and
readFileSync then followed it outside the root. Now lstat-checks for a
regular file and skips anything else with REFACTOR_FILE_UNREADABLE, so one
bad path skips one file and the run continues.

LOW — the new execute:post dispatch contract showed the gsd_run example
before the rule requiring ref.command be validated first. That prose is
executed by an agent, so textual order is execution order. Reordered.

Refs #1953

* fix(#1953): make the analyzer able to see TypeScript at all

Found by running the shipped analyzer over its own source: it reported
functions=1 for a 940-line module with 24 function forms. A return-type
annotation or a generic parameter list made a function invisible —
`function f(a): number {}` and `function f<T>(a: T): T {}` both detected as
zero. Since gsd-core is written in .cts and the capability declares
.ts/.cts/.mts analyzable, the feature silently found nothing in this repo's
own primary language while reporting success. A safety net that reports
"all clear" because it cannot see is worse than no safety net.

All 98 tests passed over this, because every fixture was plain JS — the
exact failure the test matrix's own "assert against the shape production
uses" warning describes. Adds a TypeScript-shapes suite covering return
types (including unions, generics, object literals and type predicates),
generic parameter lists (constrained and defaulted), export/async/generator
combinations, annotated arrows, class-method modifiers, and optional/
default/rest params — plus the two traps: an overload signature has no body
and must not count, and `a < b && c > d` is a comparison, not a generic.
Detection now reports 24/37/21 functions for the three source files, which
matches a hand count exactly.

Also from review:

- The strict-mode ledger dedup identified entries by parsing a prose
  description string. That is banned by CONTRIBUTING's raw-text-matching
  rule and was a real bug: the "exactly one window per untriaged proposal"
  guarantee rested on prose matching, so rewording a description or editing
  WINDOWS.md by hand silently produced duplicates. Now matches structurally
  on kind + phase + file + line.
- A property test asserted on the stripper's output text. Reframed to
  assert the same invariant through analyzeSource's score.
- nextBaseline's `candidates` parameter has been dead since the anchor
  change; removed from the signature and all call sites.
- Extracted the duplicated require-or-degrade and capability-check
  boilerplate.
- ADR-1953's Implementation bullet still named a `refactor.ship-gate` in
  check-command-router.cts — a leftover from the design cut D6 rejects.
  That file is untouched and no such gate exists. Removed.

* fix(#1953): keep execute-phase.md under its byte ceiling; un-vacuum the large-file test

Five of the seven remote-runner failures were one cause: the execute:post
dispatch contract, written out inline, grew execute-phase.md 1876 bytes
(93,400 -> 95,276) against a frozen PRE_PHASE6 ceiling of 93,600. A drift-ack
does not clear that — tests/phase6-capstone-conformance.test.cjs and
tests/fix-2285-claude-orchestration-wiring.test.cjs assert the file is
literally under the cap.

The contract now lives in gsd-core/references/loop-hook-dispatch.md, which
already claimed to be the point-agnostic dispatch reference and already
documented ref.skill and ref.agent. It gains the ref.command shape, its
in-context validation rule, the advisory-by-construction statement, and a
note that a point whose workflow hand-rolls one kind is not implementing
this contract. execute-phase.md now defers to it in one line: 145 bytes of
growth, 55 B of headroom under the cap. Better placement than the first cut
— the reference was overstating its coverage, and this makes the claim true
rather than duplicating prose next to it.

Acknowledged by appending to tests/emitted-drift-acks/2930-*.json rather
than a new 1953-*.json: two ack sources may never name the same path, and
that fragment is already the accumulating ack for this file.

Sixth and seventh failures: analyzesLargeFileWithinBounds tripped its own
vacuity guard — the fixture generated ~480 KB against a `> 500000` assert,
so the guard fired and the three assertions after it never ran. The test
has been vacuous since it was written. The matrix row specifies ~1 MB, so
N goes 8000 -> 20000 (1.17 MB, 17% margin) and the guard to > 1_000_000.
Verified by reproducing the exact body against the compiled module: 1168888
bytes, 118 ms, all four assertions hold.

Refs #1953

* fix(#1953): fold the execute:post step deferral into the existing resolve line

The remaining two failures were one test: execute-phase.md carries a SECOND,
tighter assertion than the 93,600 ceiling — `<=93400`, which is exactly its
current size. The file cannot grow by a single byte. My previous fix got it
under 93,600 but not under 93,400, so it still failed. ("H." in the report is
just the parent describe of that same test, not a separate defect.)

Rather than add a paragraph, the deferral now REPLACES the existing hook
resolution line. It read:

  Resolve active step hooks from `EXECUTE_POST_HOOKS_JSON` where
  `kind == "step"` and `ref.skill == "code-review"`.

which is the bug itself written down — only code-review was ever dispatched.
It now reads:

  Dispatch each `kind == "step"` hook per
  @gsd-core/references/loop-hook-dispatch.md. For `code-review`:

The following prose already begins "If no active code-review step hook
exists", so it reads correctly and the code-review handling is untouched.
Net effect on the file is -11 bytes: 93,400 -> 93,389, under the margin
assertion rather than merely under the ceiling.

That also removes the need for a drift-ack: the file shrank, so there is no
growth to acknowledge, and the append to the shared 2930-*.json fragment is
reverted. Leaving it would have shipped a claim of "145 bytes of growth"
that is no longer true, on a file six other issues share.

The test's own comment states the principle this ended up honoring: "the host
loop must stay small — optional-feature detail belongs in the capability
fragment, not the host workflow." Putting the dispatch contract in the
reference rather than inline is that rule, applied.

* fix(#1953): keep the code-review hook literal the workflow test requires

tests/code-review.test.cjs extracts the <step name="code_review_gate"> block
and asserts it contains `ref.skill == "code-review"` verbatim. The previous
commit replaced the line carrying that literal, so the token vanished and the
test went red — a fair assertion: code-review IS the bespoke branch there and
the workflow should still name it.

Restored inside the same one-line deferral, which now reads:

  Dispatch `kind == "step"` hooks per @gsd-core/references/loop-hook-dispatch.md.
  `ref.skill == "code-review"`:

93,396 bytes — still under the `<=93400` margin assertion and 4 bytes below
the base, so the file continues to shrink rather than grow.

Because three consecutive runs were each reddened by a different assertion on
this one file, this change was verified by sweeping ALL of them at once rather
than one run at a time: every test under tests/ that reads execute-phase.md or
references/loop-hook-dispatch.md was located by resolving its path constants,
and each content/size assertion was evaluated directly against the working
tree — 22 assertions, plus two real executions (gen-section-manifest --check,
and emitted-attribution's full real-tree differential). All pass.

That sweep also confirms the earlier judgement call: the net change to
execute-phase.md is a SHRINK, and the size ratchet only gates growth, so
reverting the append to the shared 2930-*.json ack fragment was correct — an
ack would have been both unnecessary and factually wrong.

* docs(#1953): add the missing how-to for acting on a refactor proposal

Reference and explanation shipped (COMMANDS.md, CONFIGURATION.md,
FEATURES.md 159, ADR-1953) but the Diataxis how-to quadrant did not, and
that is the one a user reaches for. CONTRIBUTING's required-docs table is
'new command -> COMMANDS.md + FEATURES.md', so CI was green on a gap.

Enabling this feature is genuinely multi-step and no single page walked it:
turn it on, tune the threshold, understand advisory vs strict, discover
that strict needs a SECOND toggle on a DIFFERENT capability, and know what
to do when a proposal appears. The two-toggle subtlety in particular was a
footnote in a config table; here it is a section with both commands.

Follows the shape of its closest siblings, resolve-edge-coverage-findings
and resolve-prohibition-findings — both 'the loop surfaced a finding, here
is what to do with it'. Includes a reason-code table for the silent cases,
since the analyzer is deliberately quiet in six situations and a user who
expected a proposal needs to tell 'nothing to report' from 'could not look'.

Indexed from docs/README.md beside the other loop how-tos.

Closes acceptance criterion 5, which I had wrongly marked satisfied.

refactor.trigger_strict records an untriaged proposal as an open deviation
window, but a ship only STOPS if workflow.windows_enforce is also on — a
toggle owned by the broken-windows capability that this feature neither sets
nor requires. So a user could enable strict, believe ship was gated, and find
out otherwise at ship time.

The split itself stays: requires:["broken-windows"] would force-install the
ledger on advisory users who never enable strict, and a ship:pre gate of our
own would never fire because ship.md has no generic ship:pre gate dispatch.
What was missing was discoverability, so that is what this fixes.

`refactor evaluate` now emits a typed REFACTOR_STRICT_NOT_ENFORCING warning,
naming the exact remediation command, whenever strict is on and either
workflow.windows_enforce is off or broken-windows is unavailable. It fires
only on a run that produced a candidate — with nothing to block on there is
nothing to warn about, and warning every run would be noise.

Reads workflow.windows_enforce through the same resolveConfigKey walk the
router already uses for its own keys rather than a second config reader.
Four tests cover the matrix: strict+enforce-off warns, strict+enforce-on does
not, strict+ledger-absent warns, strict-off never warns.

Also corrects a user-facing message in this same file that told the user to
run `gsd-tools config-set` — the wrong form. docs/CONFIGURATION.md and the
broken-windows capability both use `gsd config-set`, and gsd-tools is invoked
as `node gsd-tools.cjs`, so the bare form may not resolve. The two adjacent
messages in this file now agree.

Docs told readers to type the colon form, which no runtime registers -- 18 of
19 runtimes use slash-hyphen and the 19th uses shell-var -- so anyone copying an
example got an unrecognized command. Swept 178 occurrences across 53 files,
locale mirrors included so they do not re-diverge from English.

The colon form is a source-authoring token, not a user-facing one: install-time
converters key on it to produce the hyphen form runtimes actually register. So
the sweep is scoped, and three things are deliberately left alone:

- ADRs, which are a historical record; editing their prose falsifies what was
  written at the time.
- The legacy release-notes archive, pending a maintainer decision on whether it
  follows the same historical carve-out. Excluding it keeps a later reversal
  additive rather than a revert.
- Source artifacts under commands, workflows and agents, where the colon form is
  load-bearing. Rewriting those would break the installed-skill guarantee across
  every runtime -- the single largest hazard here.

The plugin namespace form is a real, separate token and survives untouched.

Adds a lint enforcing exactly that boundary, since the correct form genuinely
differs by directory and nothing previously caught the drift.

Also fixes a hardcoded colon form in the capability-matrix generator. The sweep
alone would have left the generated matrix disagreeing with the template that
produces it, so the fix is at the source and the output regenerated.

Adversarial review caught three lines where the sweep rewrote a citation of the
literal YAML name: key from a source command file. That key genuinely is the
colon form -- this change's own carve-out logic says source-authoring tokens keep
it -- so the docs ended up misquoting the real files. One of the three is an
acceptance-checklist assertion, which the sweep turned into a false statement.

Restored the three citations to match their sources verbatim, surgically: where a
line carried both a name: citation and a real reader-facing slash command, only
the citation reverted and the command stayed corrected.

The guard needed the same distinction, or it would have flagged the restoration
and reddened the build: a gsd:<cmd> token preceded by name: is a citation of a
source token and is now permitted. The exemption is deliberately narrow -- a bare
gsd:<cmd> anywhere else still fails -- with a test pinning that narrowness.

Also makes the detection case-insensitive. Review found /GSD:next slipped through
silently; no such casing exists in the tree today, so this closes a latent gap
rather than fixing a live one.

Swept the whole tree for further corrupted citations: none beyond the three.

Invariant #3054 banned the literal /gsd-next from user-facing docs because it
named a retired workflow-advance command. But commands/gsd/next.md is a live
command -- the state-aware smart-entry launcher -- and this issue requires docs
to use the hyphen form every runtime actually registers. Both could not hold for
this one command, so docs had been sidestepping the ban by keeping the colon
form, which is exactly the defect this issue exists to remove.

FEATURES.md already recorded the reassignment: the hyphen form "is not the
retired workflow-advance command; it is reserved for the state-aware smart-entry
launcher. Workflow advancement remains under /gsd-progress --next." With that
reassignment the invariant's premise is obsolete and the guard now contradicts
the documented command form, so it is retired with a comment recording why
rather than deleted silently.

next is now swept like every other command, and the earlier exemption added to
the new guard is removed so nothing is special-cased.

Four citations of the literal name: frontmatter key stay in colon form, because
the source file really does carry name: gsd:next and a doc quoting it must
reproduce it verbatim. Two of those lines were reworded to say which side is the
frontmatter key and which is the slash command, since they previously conflated
the two.

Verified the retired scan would now genuinely fail against this tree -- the
conflict was real and resolved, not dodged.

security-model.md, USER-GUIDE.md, ARCHITECTURE.md, COMMANDS.md,
FEATURES.md, and gsd-planner.md's STRIDE template (+ ja-JP mirrors)
described the pre-ADR-0656 design: slopcheck as the install-or-degrade
gate, with unavailability degrading every package to [ASSUMED].
ADR-0656 inverted this months ago — registry-API verdicts (npm/PyPI/
crates.io) are the gate; slopcheck is an optional escalate-only adapter
that no shipped configuration wires. Verified every replacement claim
against src/package-legitimacy.cts (checkPackages, classifyPackage,
lookupNpm/lookupPypi/lookupCrates) via Memtrace before writing it, so
the corrected prose matches the live implementation rather than
restating the ADR from memory.

Restored docs/explanation/security-model.md:79-84 (and its ja-JP
mirror) to original wording after an orthogonal spec review caught
that an earlier draft had edited the "Why WebSearch packages are
always [ASSUMED]" paragraph — inside the range issue #2775 explicitly
named as correct and to leave alone.

The ja-JP mirror was missing the closing clause present in the
corrected English original ("its absence leaves registry-API verdicts
intact rather than downgrading everything to [ASSUMED]") — added for
parity. This completes the ja-JP mirror the issue's acceptance
criteria named explicitly.

zh-CN/ko-KR/pt-BR (not named by #2775, but carrying the same stale
design) get the mechanical portion of the same fix: command-string
swaps, table headers, ARCHITECTURE.md diagram labels, and technical-
term swaps that reuse a word already attested elsewhere in the same
file (合法性/적법성/legitimidade for "legitimacy") — surrounding prose
untouched. The remainder in those three locales — full-paragraph
rewrites of the corrected degrade-path mechanism, deleted "External
dependency" bullets, and "manually install slopcheck" code blocks —
needs prose composed by a fluent speaker of each language and is filed
as open-gsd/gsd-core#3002 with an exact file:line inventory.

* test(#2775): acknowledge gsd-planner.md byte growth from the STRIDE-row fix

agents/gsd-planner.md grew 14 bytes (49309 -> 49323) from the STRIDE

declared complete. Corrected to "正当性判定" (legitimacy verdict),
matching the term already established in ja-JP/explanation/
security-model.md and ja-JP/USER-GUIDE.md. This was the only
remaining ja-JP gap; a full sweep for the transliterated form across
docs/ja-JP/ now returns zero hits, and the ja-JP mirror is genuinely
at parity.

docs/ko-KR/USER-GUIDE.md:398's heading "슬롭체크 판정:" had the same
transliteration problem. Fixed inline to "적법성 판정:", reusing the
적법성/legitimacy word already attested two lines below in the same
table. A parallel sweep of zh-CN and pt-BR found no transliterated
forms of "slopcheck" in either locale. The remaining transliterated
occurrence in ko-KR (USER-GUIDE.md:406, the lead-in to the
pip-install code block) needs prose composition like the rest of that
block and is added to open-gsd/gsd-core#3002's inventory.

query instead of hand-enumerating them, which also retires the unanchored
grep that matched --agy inside --antigravity.

Documents the previously absent reviewer body and hostBehaviors field in

Review found the flag arm was file-scoped, so the forwarding row that lists
every flag in its third cell satisfied it on its own. Deleting a lane's own
reviewer-table row -- the #2781 regression this gate exists to prevent --
therefore passed undetected.

Arm 4 keys on the FIRST table cell, which separates a lane row from the
forwarding row structurally and in every locale. Regression test included.

* fix(#2800): shape-filter the flags subcommand output

All three consumers read review-lane flags through an unquoted command
substitution so the output word-splits into loop items. Phase 2 admits
third-party overlay lanes, so an overlay flag containing whitespace would
inject a second loop item and one containing a glob would expand against
the cwd. Emit only well-formed flags so neither reaches the shell.

* fix(#2800): remove the regex length ceiling and count only prose mentions

Review found two real defects in the docs parity gate.

The never-throws contract was false: building a RegExp from a declared flag
or section title throws SyntaxError past ~100k chars, and Phase 2 admits
overlay lanes whose declared strings are untrusted in length. Every one of
these matches is literal, so String.includes replaces the regex outright,
which also deletes escapeLiteral and the llama.cpp escaping it existed for.

Arm 1 was context-blind: a flag mentioned only inside a fenced example or a
commented-out row counted as documented. Both are stripped before matching.

Also advertises all 13 lane flags in the argument-hint and corrects a stale
eleven-lane count in the slug grammar note.

* test(#2800): repoint the convergence suite off deleted workflow text

The derived flag loop deleted the literal per-flag grep lines four tests
matched on. Two of those failed loudly. The behavioral and property tests
failed SILENTLY instead: their end marker no longer resolved, so the parse
block extracted empty and both passed vacuously, and the property test's
gsd_run stub had a no-op default that hid it.

All now share one extractor and execute the real deployed block through a
gsd_run shim backed by the actual binary. The whitelist assertions become an
anti-parity check: re-adding a hand-written flag list must fail.

Also repairs two vacuous cases in the docs parity suite. The unreadable-doc
test called its own mock rather than the reader, and the integration test
bounded nothing, so a doc losing its marker would have been silently skipped
and still passed green.

* fix(#2800): run the derived flag loop after the launcher preamble

The remote matrix caught a real runtime bug, not a test artifact. In
autonomous.md and plan-review-convergence.md the launcher preamble that
defines gsd_run lives in a separate, LATER bash fence than the derived loop.
Each fence is its own shell, so gsd_run was undefined where the loop ran:
the command substitution yielded nothing and zero reviewer flags would have
been forwarded. Worse than the drift this epic fixes, and silent.

The whole CONVERGENCE_ARGS construction moves as one unit, because the
--max-cycles append sits between the loop and the preamble and would
otherwise have run against an uninitialized variable and then been dropped
by the relocated initializer.

Also documents all 13 lane flags in help/modes/full.md, which the repo gates
bidirectionally against each command's argument-hint.

* test(#2800): repoint the two converge suites off deleted flag literals

Both asserted workflow.includes('--codex') against the hand-enumerated list
the derived loop removed. They now assert the derivation itself, keep --all
and --text (convergence controls, still literal), and add an anti-parity
guard so re-adding a hardcoded list fails.

The lost pass-through proof is replaced with a real one: every flag the
tests used to hardcode is asserted present in the actual roster emitted by
the binary, which is the property the old assertion was protecting.

CodeQL js/incomplete-multi-character-sanitization (high) on PR #2882: the
single-pass <!--...--> strip can leave a live <!-- behind, so a join-trick
construction smuggles a commented-out row past the gate and it counts as
documented. Not an injection risk here since nothing is rendered, but it is
the exact false pass this helper exists to prevent.

Strips to a fixed point, then treats any surviving opener as unterminated so
the multi-line branch closes it on a later line. Terminates because every
pass strictly shortens the string.

* test(#2800): pin the comment-smuggling regression with a real reproducer

The obvious fixture for this class does not reproduce it: <!--<!---->-->
leaves a dangling --> rather than a live <!--, and is caught either way, so
it would have passed with and without the fix. The join-trick construction
(<!- + <!--DUMMY--> + -...-->), the <scr<script>ipt> shape, genuinely
regresses on the single-pass strip and is what the test now uses.

Phase 5b of ADR-2782. Adds the resolver and runner that let invoke_reviewers
iterate declared lanes instead of hand-authored per-CLI bash.

Five additive descriptor amendments, each forced by a lane that ships today:
- LaneHandler gains 'opencode' — the lane rebuilds its review from assistant
  text parts of a --format json stream; a plain stdout copy re-breaks #1936.
- modelConfigKey — antigravity's key is review.models.agy, not .antigravity,
  so resolving by slug silently dropped a configured model.
- defaultHost/fallbackModel — Phase 4 federated every *_host with a default of
  empty string; the real fallback only existed in the bash.
- args becomes an argv template with a closed four-placeholder vocabulary.
  Positional splicing produced 'codex --model M -o F exec --ephemeral', which
  is not a valid invocation: codex injects in the middle, twice.
- kimi-code lane, with the bounded command-capability probe (needle
  --output-format) that tells Kimi Code from the legacy python kimi-cli.

Parity gate re-pointed: the workflow-text families it scanned are the text this
phase deletes, so they are replaced by descriptor-to-registry parity plus an
anti-parity check that no bespoke leg returns.

jq, curl and external timeout/gtimeout all drop out of the review path.

Two bugs caught by an end-to-end stub run and fixed here:
- loadConfigResolved returns a provenance wrapper, not the config; using it
  directly resolved every key to undefined, which reads as 'nothing
  configured' and silently dropped every model override.
- hasBinary used shell:true with an args array (Node 26 DEP0190). Replaced
  with a PATH scan that spawns nothing at all.

Parity gate re-pointed, as agreed: the leg-marker and section-heading families
scanned exactly the text this phase deletes, so they are replaced by

that fires if a bespoke leg is ever re-added. Enum, emitting sites and the
Object.keys lock moved together.

The budget-trim helper is hoisted out of the Ollama leg: it was always
lane-agnostic, and any lane may now declare a promptBudgetKey.

Completes ADR-2782 D5. Rule 1 was recorded in the ADR as delivered by Phase 3
but was not implemented: ConsentRecord had no host field and nothing in the
tree bound one, so this phase's rule-4 comparison had no baseline.

ConsentRecord gains an OPTIONAL reviewerHost. Optional is the whole design:
isValidConsentRecord does not require it, so every record already on disk
stays valid and no re-consent storm fires (D4 rule 5). It is deliberately
excluded from disclosureSignature — the loader has no config resolver, so
folding a config-derived value in would make loader and lifecycle compute
different signatures for the same manifest and re-prompt forever.

Install resolves hostConfigKey (falling back to the lane's declared
defaultHost, which is what the invocation path uses) and records it.
Invocation re-resolves and blocks on mismatch rather than silently
redirecting. Absence allows: no record, or a record predating the field,
means nothing to compare — denying there would break every existing
local-model user on upgrade.

Adds the golden invocation-plan table (one row per shipped lane, derived from
the bash legs rather than the descriptor types) plus runner coverage for the

Two corrections found by running them:
- modelConfigKey was required; that breaks D4 rule 2, since a reviewer

- the antigravity non-zero-exit test pre-seeded the transcript, which asserted
  that a STALE entry leaks through — the exact bug the watermark prevents. The
  spawn now appends, as the real tool does.

* fix(#2799): restore agy --add-dir and the self-report prompt in the handler

Retargeting the three legacy reviewer suites off the deleted bash surfaced two
real regressions in the port, both #2176:

- --add-dir was dropped. Without it agy's permission context never receives the
  cwd repo, so the agent anchors on its own scratch dir and reviews the plan
  text in isolation — the exact failure the Review Instructions forbid. It is
  capability-probed, because an older agy rejects the unknown flag outright and
  a lane that fails to start is worse than one running on the prompt anchor.
- the prompt lost the clause mandating a REVIEWED-WITHOUT-REPO-ACCESS
  self-report, which is what makes a blind review distinguishable from a
  grounded one. antigravity now builds its own prompt variant.

Also ports the #2073 mode-2 cli.log diagnostic, which was dropped: a pinned
model that 404s exits 0 with empty stdout AND an empty transcript, so agy's own
log is the only evidence that anything failed.

The three suites now assert against the plan and the handler instead of
matching fence text, so they no longer need allow-test-rule exemptions.

COMMANDS.md gains --kimi-code and replaces the jq-prerequisite paragraph,
which is now false: no lane requires jq, curl or an external timeout. Adds the

CONFIGURATION.md records that the model config key is declared per lane rather
than derived from the flag — antigravity's is review.models.agy — and adds
review.models.kimi-code.

reviewer-instances.md now routes an instance through its lane's single
invocation seam instead of a copied per-adapter bash block, which is what lets
a cross-cutting fix reach instances for free. That required implementing the
--model/--agent/--as flags it documents; --model re-resolves through the lane's
argv template rather than splicing, so the flag lands where the lane declares
it rather than ahead of a subcommand.

CONTEXT.md glossary gains both new modules.

Two confirmed bugs, both reproduced before fixing:

- resolveLanePlan was not total. An openai-http lane with a missing or
  non-object invoke dereferenced inv.hostConfigKey and threw, contradicting
  the module's own documented contract; the spawn branch guarded correctly and
  the http branch did not. The CLI seam resolves every selected lane in one
  map, so one malformed overlay manifest would have aborted the whole review
  rather than dropping its own lane. Guarded, plus a per-lane try/catch at the
  seam so a throw can never take down siblings.
- A reviewer-instance model was silently dropped for any lane declaring
  modelConfigKey null (cursor, qwen, coderabbit). reviewer_instances validates
  that cli is a known slug but never that the slug accepts a model, so a user
  could configure one, get a clean run, and never learn a different model
  reviewed their plan. Now warns explicitly.

Two hardening fixes:

- The slug is concatenated into artifact paths, so LANE_SLUG_RE is enforced in
  the resolver rather than inherited from a validator that does not run on this
  path — the module documents itself as the overlay-manifest trust boundary, so
  it should not depend on someone else having checked.
- normalizeHost mangled a scheme-less value: new URL('localhost:11434') parses
  with an empty hostname, so it became 'localhost://11434' and was compared and
  requested as if real. An empty hostname now means not-a-URL.

Also documents the one gap that cannot be closed here: the antigravity
watermark is keyed by workspace, so two concurrent reviews of the same repo
share a transcript. agy exposes no per-invocation id to filter on, so the
handler now states which half of its never-stale guarantee actually holds.

review.md that this phase deletes; each protects a real invariant, so each is
retargeted onto the plan, the runner or the handler rather than removed.

Three real defects surfaced by doing so:

- effort args never reached ANY lane. model-resolver.cjs exports no
  resolveExecution, so effortFor silently returned [] every time. Restored by
  calling the same bounded resolve-execution query the bash legs used — and
  NOT with --raw, which prints the resolved effort rather than the picked
  field, so claude got 'low' instead of '--effort low'.
- the timeout guidance lost 'a silent empty output is a timeout kill, not a
  crash' — the operator note that exists because of the Codex 0xc0000142
  misdiagnosis. Restored.
- the opencode handler dropped EMPTY assistant text parts. The shipped jq was
  , and  only substitutes for false/null — an empty
  string is truthy in jq and contributed a blank line. Found by a property
  test shrinking to ['', ''].

The opencode property suite no longer spawns jq at all, which deletes the
#2099 hang mechanism it was architected around rather than mitigating it.

* fix(#2799): register the two new generated modules, and untrack them

The remote runner caught build output committed to git. Both new modules
compile from src/*.cts into gsd-core/bin/lib/*.cjs, and every sibling generated
that way is gitignored and eslint-ignored (ADR-457) - including Phase 1's own
review-lane-descriptor.cjs. Mine were neither, so repo-invariants' "each
bin/lib/*.cjs is linted xor ignored according to migration state" failed.

Registered both in .gitignore and eslint.config.mjs alongside the Phase 1
module, and dropped them from the index. Nothing about the shipped behaviour
changes; the artifacts are rebuilt by build:lib.

This is the new-.cts-module registration ripple, and it is the one part of it I
had not completed - the CONTEXT.md glossary and the inventory manifest were
already done.

   lane passed the SAME commit in 12.7s. A full install measures 13-30s idle,
   so 60s was under 2x headroom and shrinks with every file added to the
   payload. Raised to 120s, matching the heavy case already in this file.

  reversible inserts none; costly is flagged but never blocks
- the taxonomy defaults to reversible when unsure (checkpoint-fatigue guard)

- --no-reversibility-gates parses to REVERSIBILITY_GATES=false, is injected

- the override suppresses the gate but still persists the rating
- cmdVerifyPlanStructure accepts every rating and the absent case

Prose-contract assertions are Red until the implementation lands. The

  checkpoint insertion for intentionally-unattended runs while still
  recording ratings -- the override changes what stops the run, not what
  the plan remembers.

Single taxonomy, not two: references/thinking-models-planning.md #4
already shipped a binary REVERSIBLE/IRREVERSIBLE classification and is
loaded by both gsd-planner and gsd-plan-checker. It is rewritten onto the
canonical three-level vocabulary and now points at planner-reversibility.md
as the taxonomy owner, with a parity test that fails if the surfaces
diverge (DEFECT.GENERATIVE-FIX-DIVERGENCE).

agents/gsd-planner.md sat 47 chars under the 49152 LARGE_CAP, so the
checkpoint DO/DON'T guidance was relocated verbatim into
planner-antipatterns.md -- already @-referenced from the same section for
the same topic, so the planner still loads it and nothing was dropped. A
test guards the relocation against content loss.

Security — the rationale is untrusted input (ADR-1577). It originates in

path. planner-reversibility.md and the discuss-phase template now state
it is data and never instructions, and name the </reversibility>
early-termination hazard explicitly -- a rationale that closes its own
element injects sibling structure the executor reads as real tasks.
Four tests guard it.

Correctness 1 — nothing machine-enforced the feature's own promise: a task
rated one-way with no preceding checkpoint:decision validated as fully
clean, so a planner error silently reopened the gap this feature exists to
close. cmdVerifyPlanStructure now warns on an ungated one-way rating. A
warning, not an error: <reversibility> stays additive and the plan stays
valid. Four tests cover ungated (warns), gated (silent), still-valid, and
reversible/costly never flagged.

Correctness 2 — pass-always test. The --no-reversibility-gates parse test
substring-matched the whole workflow file, and plan-phase.md prose mentions
both tokens in one sentence, so it passed with the bash conditional
deleted: it was testing the documentation, not the parser. Now scoped to
the fenced bash blocks and matched as one physical line, with a negative
control confirming prose alone cannot satisfy it.

Correctness 3 — costly had no itemized emission rule, only one-way did, so
two agents could diverge on whether to tag costly at all.

Correctness 4 — template convention break: the example ratings were bare
while every sibling field uses [...] to signal substitution, inviting an
LLM to copy one-way/costly forward as boilerplate. Now bracketed.

Correctness 5 — latent false-green: .includes('reversible') also matches
inside irreversible/irreversibility, which appear in anti-pattern
prose, so a surface that dropped the real taxonomy entry would still pass.
Now word-boundary matched.

ADR-857 phase-6 ceiling — the first gsd-test run caught plan-phase.md
1216 bytes over its frozen 94519 ceiling (it had 49 bytes of headroom on
next). The ceiling may only rise for privileged host machinery, and
reversibility gating is optional-feature logic, so the wiring was slimmed
to its minimum and the explanatory prose moved to the reference files the
planner already loads. plan-phase.md is now 94400 bytes -- 119 under the
ceiling and 70 bytes SMALLER than on next, so the host loop shrank while
gaining the feature, which is what phase 6 ratchets toward. The tracer
contract (tests/tracer-bullet.test.cjs) is unchanged.

Lint — fixed an unnecessary non-null assertion in verify.cts and a
CRLF-fragile bare \n regex in the new test (DEFECT.WINDOWS-CRLF-TEST-
PORTABILITY, the #1658/#1668/#2206/#2449/#2450 class).

four cases: gated one-way (valid, silent), ungated one-way (valid, warns),
costly (valid, silent), absent (valid, silent).

Not a product defect: the validator's every-task contract is intentional
and pre-existing, and docs/reference/plan-md.md scopes its required-element
list to type=auto/tracer only because those are the elements a planner must
author, not because checkpoints are exempt from <name>.

the namesRating helper built its regex with `rating.replace(/[-]/g, '\\-')`,
which escapes the hyphen but not backslash, so the escape was incomplete.
It was also unnecessary: `-` carries no special meaning outside a character
class. Replaced with a complete metacharacter escape (backslash included).
Word-boundary behavior verified unchanged across all three ratings — notably
that "irreversible" prose still does not satisfy a "reversible" match, which
is the false-green this helper exists to prevent.

Prompt injection scan — the checkpoint fixture used the human-verification
child element inside <verify>. That tag name is a fake-instruction-boundary
pattern in scripts/prompt-injection-scan.sh, and the scan runs over changed
files, so copying the shape from tests/verify.test.cjs (unflagged only
because it is not in this diff) tripped the gate. Switched to the documented
plain-prose <verify> form.

The first attempt at that fix failed the same gate a second time: the
comment explaining the collision quoted the offending tag literally. The
comment now names it in prose instead — the scanner does not care whether a
match is code or commentary, which is the whole point of the
DEFECT.PROMPT-INJECTION-SCAN-COLLISION note in CLAUDE.md.

Verified locally before push: scan reports 0 findings across 57 changed
files, eslint clean, and both fixtures still validate as designed (gated
one-way silent, ungated one-way warns, neither errors).

The Windows shard 1/3 job timeout was traced to the sharding layer, not to
this PR's assertions — see #2472. Two contributing factors were this file's
own, and are fixed here.

1. tests/test-timings.json had no entry for reversibility-tagging.test.cjs,
   so scripts/run-tests.cjs weighted it at the table's median fallback
   (~315ms) for LPT chunk packing. It actually measures 5595ms — an 18x
   under-weight. Recorded the measured value from the green gsd-test run
   (max across the node22/node24 lanes, per gen-test-timings.cjs's
   convention). Only this one entry: a full regen churns 634 entries of
   run-to-run drift, and the table is explicitly advisory and un-gated, so
   a 637-line diff does not belong in a feature PR.

2. Each verifyPlan() spawns gsd-tools, which dominates this file's cost.
   Spawns cut from 9 to 6 with no coverage lost:
   - the ungated-one-way warning and its stays-valid assertion now share
     one plan instead of building the same plan twice;
   - the reversible/costly never-flagged-as-ungated test was strictly
     subsumed by the additive suite, which already runs those two ratings
     ungated and asserts no /reversibilit/ warning at all — and the gate
     warning's text contains both "reversibility" and "one-way", so the
     broader assertion catches it. It only re-spawned gsd-tools twice to
     prove the same thing.

Both are symptom fixes. The shard imbalance itself (19/11/10 minutes
against a 20-minute cap, from a cost-blind round-robin partition that also
reshuffles downstream files whenever one is inserted) is tracked in #2472.

earlier revision carried purely to dodge the prompt-injection scan; a
checkpoint task has no <verify> requirement at all, so the workaround is
moot.

Verified against the real gsd-tools CLI across all four cases: gated one-way
(valid, silent), ungated one-way (valid, warns), costly (valid, silent),

  the gate out of the registry's default ship:pre resolution so existing
  loop-hooks-ship-pre-e2e structural assertions (exactly 1 gate, capId
  'security') stay valid; users opt in via
  gsd config-set workflow.windows_enforce true.
- drop activationKey (security doesn't have one either; workflow.* key
  doubles as the activation toggle).
- regenerate docs/reference/capability-matrix.md to include broken-windows
  (capability-matrix-sync test).
- regenerate tests/fixtures/golden-install-parity/*.json (18 runtimes) —
  installer now emits the new capability + lib file.
- update CONTEXT.md, docs/COMMANDS.md, docs/FEATURES.md, ship.md,

Tracking-only by default; enforcement is opt-in. Acceptance criterion

* test(#1950): update ship:pre structural invariants for 2-gate registry

- loop-hooks-ship-pre-e2e: the registry now declares 2 gates at ship:pre
  (security + broken-windows), regardless of activation. Activation tests
  above still pin security-only or empty behavior via fixtures; these
  structural tests pin the REGISTRY shape, which has 2 gates as of #1950.
- workflow-size-baseline: ship.md 27928→27945 (workflow.windows_enforce
  rename added 17 bytes).

* fix(#1950): review H1+H2+M1+M2+M3 — fence-injection, EACCES fail-closed, cleanup, strict line, stryker

Adversarial isolated review (Step 6.3) found 2 HIGH findings that block
the PR and 3 mediums. All addressed:

H1 (HIGH): description containing the markdown 3-backtick fence would
terminate the ledger's JSON code block early inside JSON.stringify output
(JSON doesn't escape backticks), corrupting the file and bricking the
next parse. Fix: use a 4-backtick fence (json ... ) which
JSON.stringify cannot produce on its own, AND validate that no entry
text field contains a 4-backtick run (reject at append time with new
WINDOWS_INVALID_TEXT reason code). Locked by a regression test.

H2 (HIGH): readLedgerOrNull swallowed ALL fs errors as 'no ledger',
silently returning open_count:0 on EACCES/EPERM/EIO. The ship gate
would then pass on an unreadable ledger — the precise vector the
workflow doc claims is impossible. Fix: only ENOENT returns null;
every other fs error propagates as WINDOWS_LEDGER_MALFORMED so the
gate blocks and the operator sees a real diagnostic. Locked by a
regression test that chmod 000s a ledger with open_count=1 and
asserts the result is never a false-green 0.

M1: writeLedgerAtomic left an orphaned .tmp file on rename failure.
Wrapped renameWithRetry in try/catch with best-effort unlink.

M2: validateLine silently coerced 'abc' → NaN → null, hiding type
drift. Removed the line === 0 special case (was undocumented) and
made the error message match the strict check. Now any non-positive-
integer line value throws, including strings.

M3: tests/broken-windows.test.cjs (with its fast-check property test)
was not in stryker.config.mjs DEFAULT_TEST_CMD — Stryker would mutate
src/broken-windows.cts but no test would catch the mutations,
producing false surviving-mutant scores. Added to the list.

L1 (dead throw e after error()), L7 (line boundary tests, H1/H2
regression tests, 4-backtick CLI test) also addressed.

gsd-test v4 caught two issues:

CodeQL flagged the markdown-table cell escaper:
  String(s ?? '').replace(/\|/g, '\\|')
— it escapes pipe but not backslash first. A description containing '\|'

'cell separator', splitting the column.

Fix: escape backslash FIRST (each \ → \\), then pipe (each | → \|).
Now a description with '\|' renders as '\\\\|' (literal '\\' + escaped
pipe), which markdown renders as a single '\|' inside the cell. The JSON
code block (the parse source-of-truth) was already correctly escaped via
JSON.stringify; only the display-only table was affected.

Locked by a regression test that:
1. Verifies the JSON block reparses with the description intact.
2. Walks the rendered table row counting unescaped pipes — must be
   exactly 11 (the row separators for 10 cells), proving no in-cell
   pipe added a split.

scanEntropyAnomalies + shannonEntropy were dead exports with zero
production callers — the live hooks (gsd-prompt-guard.js,
gsd-read-injection-scanner.js) inline their own pattern subsets for
hook independence and never called these functions.

- Remove scanEntropyAnomalies + shannonEntropy from src/security.cts
- Remove scanEntropyAnomalies test block from tests/security.test.cjs
- Correct REQ-SCAN-INJ-02/-03 in FEATURES.md (EN/zh-CN/ja-JP) to
  describe what actually runs live (injection patterns, invisible
  Unicode) vs CI-only (base64-decode, codebase scan)

Adds the discoverability-registry surface for issue #2182: JSON-sourced
capability/eos catalogs, a pure schema/vocab module (registry-schema.cjs)
with the ADR-857 loop points + ADR-1239 axes, thin validate/gen CLIs,
the registry-entry PR template, README spec, and CONTEXT.md glossary terms.

The three pure functions (isValidGsdRange/validateEntries/renderMarkdown)
are stubbed here so the comprehensive test suite fails first (red), per the
feature-implementation red-first directive; the next commit implements them.

4483300253 fix(#2072): thread resolved model into routed-agent spawns (assumptions-analyzer, code-reviewer, code-fixer)
model_overrides / models.<phaseType> were silently inert for gsd-assumptions-analyzer,
gsd-code-reviewer, and gsd-code-fixer on Claude Code: resolveModelInternal honors them,
but the workflows spawned these agents with no model= param, so the resolved value
never reached the Agent tool and the agents inherited the session model — no warning.

Fix — thread each agent's resolved model at every spawn site (the established
plan-phase pattern; the architecture-consistent Claude mechanism, since 13 other
agents already thread their model):
- discuss-phase-assumptions.md: `resolve-model gsd-assumptions-analyzer --raw`
  → ANALYZER_MODEL, threaded.
- code-review.md + code-review-fix.md (re-review): `resolve-model gsd-code-reviewer --raw`
  → REVIEWER_MODEL, threaded.
- code-review-fix.md (both fixer spawns): `resolve-model gsd-code-fixer --raw`
  → FIXER_MODEL, threaded (same silently-inert bug, same file — folded in per review).
- quick.md review step: was reusing `{executor_model}` for gsd-code-reviewer (so the
  reviewer's own override was ignored); init.quick now resolves `reviewer_model`
  (gsd-code-reviewer) and the spawn threads it.

resolve-model --raw returns the bare model string (resolve-execution --raw would
return effort — wrong). The resolver maps these agents to phaseType discuss /
verification / execution, so models.<phaseType> apply too.

Scope: the three agents reachable from the two issue-named workflows + quick.md. The
wider systemic class (other agents in UNTOUCHED workflows with the same pattern) stays
documented on the issue for a maintainer-scoped structural decision (thread-at-source
vs embed-at-install like #2256), not widened here.

Docs: the stale "discuss — reserved, no subagent today" model-profile tables now list
gsd-assumptions-analyzer and the verification row includes gsd-code-reviewer, across
the English docs, the shipped gsd-core/references/model-profiles.md reference, and the
ja-JP / zh-CN / ko-KR / pt-BR locale mirrors.

Tests:
- tests/model-resolver.test.cjs: #2072 acceptance — model_overrides and
  models.discuss/verification/execution resolve for all three agents.
- tests/model-routing-spawn-threading.test.cjs: every spawn of the three agents threads
  a resolved model (fails pre-fix); a header-precise parity guard fails the suite if a
  new un-threaded spawn of any of them regresses.
All 16 golden-install-parity fixtures + the workflow size baseline regenerated for the
changed shipped files (4 workflows + the reference doc); bin/lib is excluded from parity.

ed79902509 feat(#2007): implement mempalace memory_mode kg_backend and replace routing (#2010)
Wire the two forward-declared mempalace.memory_mode modes so they actually
route recall/capture instead of silently behaving as `augment`:

- kg_backend: the palace temporal KG is the primary knowledge-graph source;
  native .planning/graphs/ is the fallback. Non-KG drawer recall stays additive.
- replace: recall resolves through the palace as the source of truth; native
  artifacts are the fallback.

Every mode stays onError:skip and default-resilient — an unreachable palace
degrades to native memory and GSD keeps writing .planning/graphs/, so no memory
is lost. Cross-mode .planning/graphs/ migration remains a documented open
question (PRD/ADR §17), out of scope here.

Surfaces updated (instruction-only contract): recall/capture commands (+ generated
skills), discuss/wave fragments, curator agent, capability.json schema. Docs:
how-to Step 3, CONFIGURATION, FEATURES, CONTEXT glossary. Regenerated
capability-registry, golden install-parity fixtures (mempalace hashes only),
agent-size-baseline. Added a routing-contract + cross-surface parity test.

Incidental (folded per no-defer rule): removed pre-existing unused imports
(spawnSync in capability-registry.test.cjs; fs in issue-498-package-identity.test.cjs)
that eslint flagged in/alongside the touched files.

Add a graphify.graph_path config key (.planning/config.json) that overrides
where /gsd-graphify query|status|diff read the knowledge graph, so one curated
umbrella-level cross-repo graph can serve multiple sibling projects without N
drifting ~5 MB mirror copies. Previously the graph location was hardcoded to
<cwd>/.planning/graphs/.

- src/graphify.cts: resolveGraphLocation(cwd, planningDir) honors the key
  (resolved relative to project root; absolute paths honored via path.resolve);
  falls back to the historical .planning/graphs/graph.json when unset/blank/
  non-string (byte-identical). Wired into graphifyQuery, graphifyStatus,
  graphifyDiff (snapshot travels with the configured graph via dirname), and
  writeSnapshot. Configured-but-missing -> actionable error naming the path.
  Build stays project-scoped (skill hardcodes the cp dest); umbrella graph is
  built in the umbrella project, sub-projects only READ it.
- config-schema.manifest.json: register graphify.graph_path in validKeys.
- tests/graphify-graph-path.test.cjs: boundary matrix (unset byte-identical,
  set+present reads configured graph not default, set+missing actionable error,
  relative resolved vs project root, blank treated as unset, snapshot alongside
  configured graph, diff from configured dir, build project-scoped) +
  VALID_CONFIG_KEYS registration.

--gemini now prints an explicit deprecation notice citing the 2026-06-18 sunset and redirects to --antigravity (no silent alias, per the issue's Hyrum's-Law rejection). Antigravity is preserved throughout: its GEMINI.md contextFileName, .gemini/antigravity config home, the shared convertGeminiToolName/claudeToGeminiTools tool vocabulary, and the 'gemini' hookEvents dialect it declares. GEMINI.md retargeted as Antigravity's context file.

the phase as body 'Phase: N' and total_phases/percent under a nested
'progress:' YAML object. Both came back null, so active projects
(e.g. this repo at Phase 3 / verifying) wrongly classified as
needs-first-phase.

- detectSignals now reads total_phases + percent from nested progress{}
  first, then scalar fm, then body; current_phase falls back to the
  body 'Phase:' field (parseProsePhaseField lineage).
- Add regression tests against the real schema (nested progress YAML +
  body Phase field) covering verify-pending + executing situations.

Verified against this repo: now classifies verify-pending (was
needs-first-phase). Coverage 93.25% lines / 86.99% branches.

* fix(workflow): tiered fallback when gsd-tools is broken (not just smart-entry)

Live test exposed a self-defeating fallback: when smart-entry --json
failed because gsd-tools itself was broken (missing
markdown-sectionizer.cjs), the workflow fell back to /gsd:progress —
which also depends on gsd-tools and would dead-end too.

Replace the single /gsd:progress fallback with a tiered recovery:
1. Probe gsd_run state-snapshot. If it ALSO errors, the whole tool
   layer is down — read .planning/STATE.md directly with the Read tool
   and synthesize a minimal situation + actions menu so /gsd:next stays
   useful. Surface a rebuild hint.
2. Only if smart-entry alone is missing (older gsd-core), fall back to
   /gsd:progress as before.

Matches the direct-read resilience the live agent already did by hand.

Root-cause of the intermittent `full test (windows-latest, 22, shard 1/3)`
failure. It was NOT a leaked handle (the runner's kill message guesses that,
but --test-force-exit already exits leaks cleanly). Diagnosis:

- Ran every shard-1/3 file WITHOUT --test-force-exit + a 45s kill-timer:
  zero hangs, zero leaks — every file self-exits. So no leaked handle / hang.
- CI activity profile: output kept flowing (slowly) right up to the 600.0s
  kill — a dead hang would go silent. => pure slowness.
- Per-file timing: install-minimal-hooks.test.cjs is a 4987-line / 250-case
  consolidation file doing dozens of real installs — 41s even on a fast Mac
  (much worse on the slow Windows I/O path), plus an install-heavy cluster.

Mechanism: MAX_FILES_PER_CHUNK=180 packed the whole ~171-file shard into ONE
`node --test` chunk, so the entire shard's wall-clock ran against a single
600s per-chunk backstop. On slow Windows runners that single chunk crossed
600s and was killed mid-run — an intermittent false-negative gate that also
hits `next` directly.

Fix: lower MAX_FILES_PER_CHUNK 180 -> 90 so each shard splits into ~2 chunks,
each with its own fresh 600s budget and a fresh node process (also relieves
per-process memory pressure). Verified locally: shard 1/3 now runs as
chunk 1/2 (90 files) + chunk 2/2 (81 files), 5323 tests, 0 fail. Also made the
timeout kill-message name slowness as a cause instead of asserting a leak, so
the next debugger isn't sent hunting a nonexistent handle leak.

the 4 slash/colon-namespace-leak invariant suites that had no canonical owner. Verbatim
block-scoped describe wrappers; 427 subtests conserved 1:1.

Host-env pre-check (per B2): no CLI-receiving host sets a redirecting GSD_WORKSTREAM/GSD_PROJECT
value. One folded suite (bug-3668 runtime resolver) creates an extension-less PATH gsd-tools
stub + bash -c; co-locating it with the host's chmodSync tripped local/no-unguarded-nonportable-exec,
so it's now Windows-guarded (skip on win32) matching the host suite's own bash -c guard.

Regenerates regression-name allowlist (222->182), ratchets file-count allowlist (graphify 7->6,
docs entry removed), makes 26 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456;
prunes stale ids). Repoints 13 tests/ references across CONTEXT.md, COMMANDS.md/FEATURES.md
(EN + ja/ko/pt/zh) and ADR-0002. lint:ci green.

parenthetical, and make verify-phase abstain -> human_needed (never silent-pass)
on a backstop truth it cannot confirm with explicit evidence. Truth-axis mirror
of #644's prohibition judgment-tier (ADR-550 D4).

Engine (deterministic, CI-tested per ADR-550 D5 — never the LLM verdict):
- src/probe-core.cts: truthStatement/truthVerification normalizers, projectTruths
  (conservative serializer), dispositionForUnverifiableTruth (backstop+no-evidence
  -> unverified/flagged/insufficient_spec; backstop+evidence -> green; inferable
  -> green, the over-abstention guard).
- src/roadmap.cts: coerceTruthToString now reads `statement` first so an object-form
  backstop truth is surfaced, not dropped (Hyrum backward-compat for truth-readers).

Workflow/agent/docs: plan-phase emits the structured marker (flat scalar, ADR-550
#1278); verify-phase + gsd-verifier add the abstain arm; new references/honest-verifier.md;
FEATURES/COMMANDS document insufficient_spec; ADR-550 amended (truth-axis D4 mirror).

Decisions adopted (trek-e review): insufficient_spec feeds existing human_needed with a
distinguishable reason (no new VERIFIER_STATUS); changeset Changed; round-trip parity
test; abstain-on-unconfirmed-backstop regression test red-first.

Implementation notes (deviations from the issue's proposed file list, verified live):
- frontmatter.cts needs no change — its flat parser already round-trips object-form truths.
- verify.cts needs no change — it grades artifacts/key_links structurally; truths are
  LLM-graded at the workflow layer, so consumption lives there + the deterministic helper.
- No CJS<->SDK hand-sync — the SDK seam was retired (ADR-0174); src/*.cts is sole source.

Regenerated artifacts: golden-install-parity fixtures, INVENTORY-MANIFEST, size baselines.

`insufficient_spec` backstop truth is also excluded (it is not ✓ VERIFIED and routes
to human_needed). Behavior was already correct; this tightens the wording.

verify-phase.md. (Nit-2 — a dedicated insufficient_spec_items frontmatter list — is
intentionally not taken: the current design is ADR-550-D4-conformant, the abstain

  audit.scanSeeds, which only returns unimplemented seeds for the milestone surface.

install/upgrade/remove + capability-ledger.cjs) across Phases 3-5 but never wired to a
user-facing command — gsd-tools.cjs 'capability' only handled state/set. This adds the
six subcommands, dispatching to the existing lifecycle/ledger:

- install <spec> [--integrity] [--scope global|project] [--yes] [--shared-file <rel>]…
- update [<id>|--all] [--scope] [--yes] [--shared-file]  (re-resolves recorded source)
- remove <id> [--purge-data] [--scope]  (first-party rejected)
- list [--json]  (first-party + overlay, both scopes, JSON array)
- disable|enable <id>  (activation-state alias of capability set --off/--on)

Scope→runtimeDir mapping matches capability-loader exactly (global=$GSD_HOME||home,
project=project root; caps at <root>/.gsd/capabilities/<id>, ledger at <root>/.gsd-capabilities.json).
Consent is non-interactive: --yes grants; without it an executable install aborts after
printing the disclosure and writes nothing. Best-effort reconcile before each mutation.

Tests: tests/capability-cli.test.cjs (20 behavioral, real resolver via local specs,
GSD_HOME-sandboxed) — install consent/block/usage matrix, list, update round-trip,
remove round-trip + first-party guard, disable/enable, unknown subcommand.

  stdout. Now it flushes the captured buffer before re-throwing (exit code preserved).
- cmdCapabilitySet threw via process.exit() (bypassing the capture wrapper entirely); now throws
  ExitError so the wrapper flushes — matches the repo's no-process-exit architecture.
- Regression test: capability disable <unknown> --raw must emit the JSON error envelope on stdout.

Verified: capability suite 165/165, @file/json-errors/phase 183/183, lint clean.

  cleanupCodexSkillMetadataSidecars: Codex-only (if isCodex), removes stale
  managed gsd-*/agents/openai.yaml and prunes the now-empty agents/ dir.
- Preserve user-owned dirs (gsd-dev-preferences), non-empty agents/ dirs, and
  non-gsd dirs; lstat-guard against symlinked agents/ so a delete can never
  escape the skills tree; fail-open per directory.
- Codex relies on SKILL.md alone for /skills discovery.
- Update USER-GUIDE/FEATURES docs and rewrite the #774 emission tests into
  cleanup tests.

Scope: the sidecar duplicate only. The separate multi-root (~/.agents/skills

- B2: both child spawns get a bounded timeout (30s node / 60s eslint) + 16MiB maxBuffer;

- M1: scoped verify-phase.md — the check descriptor is author-supplied for now; filed

31b822b312 fix(1259-01): close adversarial review findings — genuine enforcement, honest fail-first scope
Adversarial pre-submission review found the injected-runCheck tests masked a
non-functional real runner. Fixes:

- BL-01 (false green on vacuous test): the node-test runner now parses the TAP
  summary and requires a NON-VACUOUS pass (>=1 test, >=1 pass, 0 fail) AND a
  reported test named distinctly from the file — node --test counts an empty file
  as one passing test, so counts alone could not catch it.
- SF-01 (lint anchor never greened): the lint-rule runner now runs the project
  eslint as --format json and filters by ruleId, so plugin rules (local/*) load
  via the flat config — bare --rule cannot load a plugin. local/no-source-grep
  now genuinely greens (covered by a real, non-injected test).
- BL-02 (tautological fail-first): the runner no longer echoes the caller's
  failFirst as if confirmed. failFirst is documented as caller-ATTESTED; the
  producer requires attestation + a genuine non-vacuous pass. Machine-proven
  fail-first (needs a violation fixture) is flagged as a tracked follow-up in
  ADR-550, the changeset, FEATURES, the reference doc, and verify-phase.
- SF-02: added real-runner end-to-end tests (no injected runCheck) + pure,
  exported parse/filter helpers (parseNodeTestSummary, tapTestNames,
  eslintJsonHasRule, eslintFileResultCount) so the shipping branches are
  mutation-pinned.
- NIT-01/02: LOCATE guard rejects empty-string rule and unknown kinds.
- Hardening: spawn checks with NODE_TEST_CONTEXT/NODE_OPTIONS scrubbed so an
  ambient test-runner context cannot corrupt a verify-time result.
- Docs reconciled to the shipped behavior (no 'confirms fail-first' overclaim).

3556450b0d feat(spec-phase): surface zero-classification edge-probe requirements as unclassified candidates (#1110) (#1117)
Edge-probe now surfaces a zero-classification requirement (non-empty prose, no
shape cue matched, no `shapes` override) as a single soft `unclassified — review
manually` candidate instead of silently dropping it — the exact blind spot the
probe exists to catch. Dismissible like any edge; the `shapes: []` opt-out stays
silent; `TAXONOMY` (the closed 8 categories) is unchanged. Under `--auto` the
candidate is left `unresolved`, never auto-`backstop` (a missing shape is not
evidence an edge exists).

MemPalace (local-first memory: MCP server + CLI) into the GSD loop:
deliberate recall before discuss/plan and verbatim + temporal-KG capture
at phase boundaries. Three memory modes (augment default; kg_backend and
replace forward-declared). Master gate mempalace.enabled (default off);
every hook onError:skip, zero gates; absent/disabled MemPalace => loop
unchanged. Transport is rendered-markdown only — MemPalace runs
out-of-process, no third-party code in gsd-core (ADR-857 §7).

Capability: capabilities/mempalace/ (manifest + 2 fragments), skills
commands/gsd/mempalace-{recall,capture}.md, agent
agents/gsd-mempalace-curator.md. Registration: ns-context router,
utility cluster, KNOWN_SKILLS, help full.md, model-catalog, copilot
install list, size baselines; regenerated capability-registry +
inventory manifest. ship:post wired into ship.md (wire-on-demand).

HELD on #1196: this capability also declares hooks at discuss:pre and
discuss:post, which are structurally un-wireable until the host-loop
conformance model covers the discuss phase (discuss-phase.md is not in
HOST_LOOP_FILES). The phase6-capstone-conformance gate therefore fails
on exactly those two orphaned points by design — see #1196. Once #1196
lands, rebase onto next and the gate goes green with no further change.

827011b865 fix(#1098): guard generate-claude-md against clobbering hand-crafted files; redirect default to .claude/CLAUDE.md (#1118)
/gsd-new-project wrote a repo-root CLAUDE.md full of broad project docs,
overwriting/diluting a hand-crafted instruction file. --force was parsed but
silently dropped, and nothing guarded an existing non-GSD file.

- Guard: an existing instruction file with no `<!-- GSD:<section>-start` markers
  (hand-crafted) is left untouched; report action:"skipped". --force (now wired
  through CmdGenerateClaudeMdOptions) overwrites intentionally. The marker check
  uses /<!-- GSD:[a-z]+-start/ so a file merely documenting GSD syntax is safe.
- Redirect: the Claude-family default output is now ./.claude/CLAUDE.md (a valid
  auto-loaded project-memory location) instead of repo-root ./CLAUDE.md. Aligned
  across the handler default, config-defaults.manifest.json, buildNewProjectConfig,
  the config template, new-project.md, and cmdGenerateClaudeProfile; advisory
  read-CLAUDE.md hints in plan-phase/quick/profile-user updated. Codex still
  writes AGENTS.md.

* feat(spec-phase): spec-completeness edge-probe (#550) — relocated to gsd-core/

Rebased onto current next and relocated the whole feature from get-shit-done/ to
gsd-core/ per #615 (trek-e re-review #4, option 1). The artifact now builds to
gsd-core/bin/lib/edge-probe.cjs; all hard-coded path strings (tests, workflow
@-refs, run-tests.cjs sentinel, eslint ADR-457 ignore, .gitignore) updated.
Content conflicts in .gitignore / eslint.config.mjs / run-tests.cjs resolved.

Feature: Step 5.5 edge-completeness probe walks each SPEC requirement against a
closed 8-category edge taxonomy, proposes applicable candidate edges, and resolves
each (covered/dismissed/backstop/unresolved). covered/backstop criteria are lifted
by plan-phase into must_haves.truths, extending the goal-backward verifier's reach
to boundary edges no requirement was written for. Engine authored as strict TS
(src/edge-probe.cts, ADR-457), compiled to the gitignored gsd-core/bin/lib/edge-probe.cjs.

Folds in every prior review round on PR #584:
- RR-01..03: plan-phase resolves the phase *-SPEC.md and injects {SPEC_PATH} into the
  planner; must_haves<->Edge-Coverage quality_gate; held-out planner-contract test.
- RR-04/11: Step 5.5 invokes the compiled engine at runtime (npm --prefix-pinned,
  source-checkout-gated build fallback) instead of LLM re-derivation; the engine
  capture is exit-checked and the report JSON-validated before use (fail closed).
- RR-05..10: all six fixtures embedded + count-equality; backstop/covered require a
  resolution; Array.isArray(shapes); duplicate-resolution rejection; CLI JSON exit(2);
  per-artifact build sentinel.
- Authored-shape validation: invalid (non-empty) shapes fail closed (VALID_SHAPES).
- Adversarial-review hardening: orphan/typo resolution rejection, requirement input
  validation (id/text/shapes, duplicate id, non-array), zero-applicable guard.

Full suite 0 failures; npm run lint 0 errors; edge-probe suite 72/72.

* test(#550): RED — status×verification re-cut + probe-core engine specs

Re-cut the edge-probe resolution model onto two orthogonal axes per
ADR-550 Decision 7 (trek-e #644 comments 2026-06-03 14:36 + 14:44):

  status: resolved | dismissed | unresolved   (lifecycle, shared)
  verification: explicit | backstop | null     (only when resolved)

- tests/probe-core.test.cjs (new): behavioral specs for the generic engine
  to be extracted — validateResolution(r, validators), validateRequirement,
  analyzeCoverage(items, resolutions?, validators), byVerification rollup,
  runProbeCli I/O scaffold (injected-io unit tests).
- tests/edge-probe.test.cjs: covered→{resolved,explicit}, backstop→
  {resolved,backstop}; coverage gains byVerification.{explicit,backstop};
  proposeEdges items gain verification:null.
- 6 fixtures re-genned + re-embedded in edge-probe.md; coverage.resolved
  COUNT preserved on every fixture (closed set = resolved+dismissed; doc
  line: 'adjacency=covered + ordering=dismissed' -> 2). edge-probe.md prose
  rewritten to the two-axis model.

Fails as expected: probe-core.cjs has no source yet; edge-probe still
emits the old covered/backstop enum (27/61 edge specs red).

* feat(#550): extract probe-core seam + refactor edge-probe onto it (ADR-550 D7)

Extract the generic resolution model into src/probe-core.cts (the shared
seam the prohibition probe #644 is born on) and refactor edge-probe.cts
into its first adapter.

edge-probe keeps ONLY the edge cluster: Shape/SHAPE_CUES/VALID_SHAPES/
classifyShape/TAXONOMY/applicableCategories/proposeEdges + EDGE_VALIDATORS
{explicit,backstop}; delegates merge/rollup/CLI to probe-core. Every shipped
#584 guarantee preserved (fail-closed shapes, orphan/dup rejection, input
validation, CLI exit 2). 104/104 edge+probe-core+docs+contract specs green.

* chore(#550): register probe-core.cjs artifact in ledgers + inventory

New gitignored build artifact gsd-core/bin/lib/probe-core.cjs (compiled
from src/probe-core.cts) needs registering in every artifact ledger:

PR #584 alongside the probe-core extraction (Decision 7) it governs, so the
contract and its first implementation arrive together.

Decisions: probe packaging (3 layers); recall->precision protocol;
prohibition home = SPEC acceptance criteria + optional must_haves.prohibitions:
(truths untouched, no polarity); tiered verification test|judgment
(judgment = mode-dependent soft-gate-with-flags, never silent pass / never
hard-halt); CI tests the contract not the classifier; secure-phase ownership
seam; and Decision 7 — probe-core seam + status×verification re-cut (7a-7e),
which this PR implements.

* feat(#550): fail-closed probe-core across full status×verification + runProbeCli structural guard

Re-review #5 (trek-e) seam-hardening on the generic probe-core contract #644 inherits:

- validateResolution now enforces the 'verification is null unless resolved'
  invariant for EVERY status (not just resolved): a dismissed/unresolved
  resolution carrying a verification tier is rejected instead of merging verbatim.
- An unresolved resolution carrying a resolution/reason payload is rejected
  (was silently dropped into the unresolved count).

- coverage.resolved kept count-preserved (closed set) per the blessed migration
  contract; a new test locks that an all-dismissed run is NOT affirmatively covered

Re-review #5 (trek-e) clarity edits:

- Decision 5: annotate that only contract item (a) ships on #584 (the edge
  adapter's parse+validate test); (b)–(d) are #644 scope, matching Consequences.
- Decision 7a: correct the 'coverage.resolved is preserved (status === resolved)'
  parenthetical — the blessed/implemented semantics are count-preserved = the
  CLOSED set (resolved + dismissed = applicable − unresolved), with byVerification
  carrying the per-tier resolved-status breakdown. The old parenthetical
  contradicted the shipped count.

* test(#550): cover runProbeCli structural-guard numeric-count branch

Second-pass coverage audit found the 'coverage object present but counts
non-numeric' branch of isValidReport (built probe-core.cjs:60-61) unexercised —
the {nope:true} malformed test fails earlier at the items[] check. Add a report
with well-formed items[] + a coverage object carrying non-numeric counts so the
numeric branch is hit. No source change; closes the line gap.

* fix(#550): reject edge requirement with missing/empty text when no shapes override (M2)

The edge adapter's `text` is the classification signal and a required field, but
core `validateRequirement` left it optional, so a `{ id }` requirement classified to
zero shapes -> zero edges -> was silently DROPPED from coverage with no signal -- the
exact fail-open this feature exists to eliminate. Reject missing/empty text unless an
authored `shapes` override (incl. `[]`) opts out of prose classification.

* fix(#550): validate verbatim items in analyzeCoverage shared seam (m1)

A proposed item with no matching author resolution is rolled up VERBATIM, but its own
status/fields were never validated -- an item carrying an out-of-enum status (the dropped
"covered") or `dismissed` without a reason would be counted closed. The edge adapter only
proposes `unresolved` items, but the prohibition adapter (#644) proposes LLM-generated
items that arrive populated. An Item is structurally a superset of a Resolution, so reuse
validateResolution to fail closed. ADR-550 Decision 5 hardens this shared seam.

* fix(#550): move edge-coverage lift instruction to runtime planner surface (M1)

templates/planner-subagent-prompt.md is loaded by nothing at runtime (no @-import in
agents/gsd-planner.md; plan-phase.md spawns the planner from its own inline
<planning_context>), so the precise covered/backstop -> must_haves.truths lift instruction
this PR added there never reached the planner -- and the RR-02 contract test asserted it in
that dead file, giving false green. Move the instruction into plan-phase.md's runtime
<downstream_consumer> block (where the rest of the wire already lives), revert the dead-template
edit, and retarget RR-02 to the loaded surface with a guard against re-orphaning.

* test(#550): lock machine<->SPEC vocabulary mapping against drift (m2)

The machine contract uses orthogonal status x verification; the SPEC table renders a flat
covered/dismissed/backstop/unresolved. The migration map (ADR-550 Decision 7a) was prose-only
with no test, so the layers could silently drift. The SPEC table is LLM-rendered (no JS
renderer to round-trip), so pin the canonical bijection as code AND ground it in every doc
surface that renders the vocabulary (ADR migration clause, spec.md legend, reference mapping
table) -- a rename or remap now fails the suite.

* docs(#550): add how-to for resolving edge-coverage findings (B1)

Feature shipped reference coverage (FEATURES.md, COMMANDS.md, references/edge-probe.md) but
no how-to -- reference-only does not satisfy the Diataxis docs standard for a user-facing
capability. Add a single-mode how-to (imperative, goal-directed) walking each resolution
state (specify/dismiss/backstop/defer), the soft gate, and --auto, with taxonomy/concepts
linked out to the reference. Register it in the docs/how-to index.

* docs(#550): add Probe Core + Edge Probe glossary entries to CONTEXT.md (N1)

trek-e re-review #7 N3 (Nit): the // allow-test-rule: tokens (source-text-is-the-product,
docs-parity) differed from CONTEXT.md's canonical exemption category
'runtime-contract-is-the-product' (RULESET.TESTS.no-source-grep.exemption, CONTEXT.md:240).
All three tests assert deployed runtime-contract surfaces (spec-phase.md Step 5.5, the
plan-phase.md planner prompt, the rendered reference/SPEC/ADR vocabulary), so the canonical
category fits; each now carries a one-line justification per the ruleset format. Free-text
reason — lint behavior unchanged.

* test(#550): re-baseline plan-phase + spec-phase byte sizes for edge-probe

Rebased onto next (e4f0910d), which replaced the line-based tier-max
size ratchet (#597) with the byte-based per-file baseline guard (#1074).
The edge-probe feature legitimately grows two workflows:

Rebase onto current next dropped the prior reconcile commit (stale counts

required check passes. Post-SDK-retirement (ADR-0174/#174) successor to the
SDK-framed #70, with no SDK-specific API surface.

New pure module src/uat-predicate.cts:
- stripFalsePositiveContexts: frontmatter -> HTML-comment -> CommonMark-style
  fenced-block state machine (tracks delimiter char+length) -> blockquote,
  each a small composable step, so a `result: passed` inside frontmatter, a

flags via makeInvalidArgs. Hardened across two Codex adversarial passes
(vacuous pass, dropped failing tests, permissive verification status,
nested-fence escape, cross-line result value, masked unterminated comment) —
all fixed fail-closed. New unit + CLI-integration suites incl. a fast-check
property test; docs, CONTEXT glossary, inventory, and changeset updated.

`raw.replace(/<!--[\s\S]*?-->/g,'')`-then-`.includes('<!--')` detection in
analyzeMarkdown as incomplete sanitization (a single regex pass can leave a
residual `<!--`). Replace it with a paired left-to-right indexOf scan that
contains no `.replace()` of the comment token — CodeQL-clean and strictly
more correct (a closed earlier comment can never mask a later unterminated
one). Behaviour unchanged; 98 predicate tests + scoped docker run green.
