6b0b92674a refactor: remove dead descriptor-driven mechanisms (no remaining consumer)
Drop code whose only consumer was a retired runtime: hostBehaviors readers for
agentFileExtension, localTargetIsProjectRoot, sharedHooksDirName,
skillsManifestPrefix and skipCodexSkillsManifest; the empty
NON_REGISTRY_CONFIG_HOME_DESCRIPTORS array and live-config-guard plumbing; the
empty RUNTIME_NOTE_AUDIENCE_BY_HEADING filter; the unused resolveVersionFrom
export; and the WINDSURF_SESSION_ID workstream session key. Delete tests that
only exercised those mechanisms.

6cfa0c55d2 refactor: drop 12 runtimes, keep Claude, Codex, OpenCode, Cursor, ZCode, Antigravity
Removes kilo, kimi, kimi-code, copilot, windsurf, augment, trae, qwen, hermes,
cline, codebuddy and pi end to end: capability descriptors, installer branches
and converters (bin/install.js 14.9k -> 11.2k lines), TypeScript converters,
hook surfaces and runtime homes, review lanes qwen/kimi-code, the two pi
migrations, Kimi payload normalization in the hook guards, dead hostBehaviors
vocabulary, launcher home probes, fixtures, runtime-specific tests and the
prose that presented them as supported.

Installer output for the six kept runtimes is byte-identical to before the
prune. The Kimi tool-vocabulary tests in workflow-guard, read-guard and
read-injection-scanner are left in place pending a decision.

a9a7a328e6 refactor: hard-fork GSD -> MSD (Make Software Done)
Mechanical rename produced by scripts/msd-rename.cjs: gsd/Gsd/GSD -> msd/Msd/MSD
across contents and paths, upstream package/repo coordinates -> @golem15/msd-core
and golem15com/msd-core. Deep links into upstream history, sibling upstream
packages, the GSD-2 import feature, CHANGELOG.md and .changeset/ are kept as-is.

Hand edits on top: MSD block-letter banner and logos, LICENSE copyright line,
package/plugin identity, regenerated lockfile, install-tree fixtures, derived
registries and benchmark baseline; migration checksum baseline re-locked
(MSD keeps its own install state, so no install had applied the old sums);
sort-order and regex-escaped expectations in tests adjusted.

Batch 4 of the ad hoc timeout literal migration (epic #4445). Replaces
every bare numeric timeout/timeoutMs object-literal property across
tests/install.test.cjs, tests/install-minimal-hooks.test.cjs,
tests/fragment-single-edit-propagation.install.test.cjs,
tests/install-regressions.test.cjs, tests/install-runtime-artifacts.test.cjs,
tests/npm-integrity-gate.test.cjs, tests/faulty-deps.test.cjs,
tests/release-tarball-smoke.install.test.cjs,
tests/install-write-confinement.test.cjs, tests/plugin-manifest.test.cjs,
and tests/packaging-shipped-scripts-require-only-shipped.test.cjs with a
named constant, per eslint-rules/no-adhoc-timeout-literal.cjs. Removes
these 11 files from the rule's allowlist.

Adds one new shared constant to tests/helpers/timeouts.cjs for a
fixture-JSON value (in seconds, not ms) mimicking a Claude Code
settings.json hook-entry's own timeout field, shared across two files in
this batch. Every other new constant is file-local, each carrying a
comment explaining why its call site is a distinct operational class from
the existing shared norms even where its digits numerically coincide with
one. No src/bin file touched, no numeric timeout value changed anywhere.

Discovered while verifying PR #4575 (a full test (windows-latest) failure in an unrelated test, tests/kilo-upgrades.test.cjs's worktree-path-guard rejection test). Root cause: this hook's up-to-4 sequential git spawns (git-dir, branch, worktree-toplevel, file-toplevel), invoked inside a native-plugin's own 8000ms-capped subprocess wrapper, left zero margin for node/git startup overhead — the guard is deliberately fail-open on any timeout, so CI-load-induced latency in this chain silently downgrades a security block into an allow. Combines the first three git rev-parse calls (git-dir, branch via --abbrev-ref, worktree-toplevel) into ONE spawn instead of three, using git's documented multi-flag rev-parse output (one line per flag, in order) — verified empirically including the detached-HEAD edge case. No timeout value changed; this reduces subprocess COUNT, not any tolerance.

423f38e655 fix(#4444): honor config-set --dry-run instead of silently ignoring it (#4504)
* test(#4444): failing-first regression coverage for config-set --dry-run

config-set --dry-run is currently parsed nowhere -- routeConfigSet
(gsd-core/bin/gsd-tools.cjs) never checks args for it, and cmdConfigSet
has no dry-run parameter, so the flag is silently swallowed and the
command always writes for real. Reproduces the issue's own repro
(sequential --dry-run calls where the second's previousValue proves
the first persisted), plus coverage for validation-still-runs,
secret-masking, and the sibling unset (config-set <key> null) branch,
which has the identical defect. This commit adds the regression
coverage only; the fix lands in the next commit.

routeConfigSet (gsd-core/bin/gsd-tools.cjs) never read args for
--dry-run, and cmdConfigSet had no dry-run parameter at all -- so the
flag was silently accepted (as any unrecognized trailing argument is)
and the command always wrote for real. A second "dry run" then showed
previousValue reflecting the first one, proving it had persisted.

Threads a dryRun option through cmdConfigSet, gating BOTH mutating
branches: the null/unset path (unsetConfigValue) and the real-set path
(setConfigValue) -- the unset branch had the identical defect,
undiscovered until auditing every mutation site while designing this
fix. Each gains a previewConfigValue/previewUnsetConfigValue
counterpart that reuses the real function's exact traversal/creation
logic (_setNestedValue/_unsetNestedValue) on a throwaway in-memory
config copy that is never written -- so the preview can never diverge
from what the real write would compute. All validation (unknown key,
enum/number/boolean checks, secret masking) runs identically whether
or not --dry-run is passed; only the final write is skipped, replaced
with a `{ dry_run: true, would_update / would_unset: true, ... }`
preview payload matching the precedent established by `milestone
complete --dry-run` (#2118) and `todo complete --dry-run` (#4096/#4325).

install-minimal-hooks.test.cjs (weight=24.45, the heaviest file in the
suite) sits alone in its own chunk yet still occasionally brushed the
600000ms per-chunk ceiling on Windows -- observed on PR #4504's first
CI run for this change (passed clean on rerun, consistent with the
"legitimately too slow for the budget" cause the chunk-timeout
diagnostic already names, not a leaked handle).

Raised RUN_TESTS_CHUNK_TIMEOUT_MS's default from 600000ms to 800000ms:
~33% more margin, still comfortably below the 900000ms regen:derived
fixture timeout that fragment-single-edit-propagation.install.test.cjs
deliberately keeps ABOVE the chunk ceiling, and far under the 45-minute
job cap -- Windows shards currently finish in ~19-20 minutes total, so
there is ample headroom. Updated every dependent mirror/assertion in
lockstep (tests/helpers/emitted-runtime.cjs's duplicated
CHUNK_TIMEOUT_CEILING_MS constant, its lock test in
tests/emitted-attribution.test.cjs, the Windows-skip prose in
fragment-single-edit-propagation.install.test.cjs, and
docs/TESTING-SUITES.md's reference table) so nothing describes a stale
value.

This file's real, unrelated pre-existing cost (dated 2026-09-06, PR #4428)
is what tipped a Windows CI shard over the per-chunk timeout backstop on
PR #4504 (issue #4444's own diff never touches this file or the
installer). Rather than raise the timeout, cut the file's actual spawn
count: several describe blocks independently re-installed the IDENTICAL
runtime/scope/flag configuration just to assert different things about
the same install output. Merged each such group onto a single shared
install, with every original assertion preserved:

- --help x3 -> x1
- the three per-runtime/scope --minimal E2E loops (global, local, and
  on-disk-matches-manifest) merged into one loop over
  SKILL_RUNTIMES x [global, local]: 44 spawns -> 22
- the --minimal manifest-mode/backcompat triple-install -> one shared,
  memoized install via sharedMinimalManifestInstall()
- .sh hooks existence checks (5 tests) -> 1, executable-bit check (its
  own Windows-conditional skip) left separate
- Codex #4087 hook-helper tests (3) -> 1
- Windsurf #4087 hook-helper tests (2) -> 1
- pi shared-hooks-bundle tests (3 per scope) -> 1 per scope

Net: ~65 real installer spawns in this file down to ~29, no assertion
dropped or weakened.

77e2472ca0 enhance(#4221): replace installer Read() deny rules with a managed secret-read guard hook (#4236)
* feat(#4221): gsd-secret-read-guard PreToolUse hook + registration

Add hooks/gsd-secret-read-guard.js, a blocking PreToolUse guard on
Read|Grep|Bash that denies reads of .env, .env.<suffix> and .secrets
(the .env.example/.sample/.template/.dist templates stay readable).
Read checks file_path; Grep checks an explicit path and judges the glob
per brace alternative; Bash runs a two-pass token scan (quotes, comments,
redirects with fd digits, separators, $( )/backtick/<( ) recursion,
heredoc bodies never scanned as commands, nested bash -c/eval rescans,
git <ref>:<path> shapes) with a closed non-reading exemption set for
existence checks. Fail-open crash policy; 1 MiB commands are denied as
command-too-large; more than 64 glob alternatives as glob-too-complex.

Why: Claude Code 2.1.259 makes every `cd DIR && grep …` compound prompt
for approval whenever any Read() deny rule exists, even in auto mode. A
hook denial is not a permission rule and never arms that check. The
installer-written deny rules are retired in the follow-up commit.

Registration: hooks.json (Read|Grep|Bash, timeout 5), build-hooks
HOOKS_TO_COPY, managed-hooks-registry, runtime-hooks-surface (blocking
guard with BLOCKING_GUARD_TIMEOUT_S; Kimi ReadFile|Grep|Shell),
shell-command-projection managed sets, installer-migration-report,
OpenCode/Kilo plugin (grep tool mapping, include -> glob, dispatch),
docs tables in five locales, ADR-766 always-on list, regen:derived
fixtures, and a new table-driven unit suite.

* test(#4221): pin the secret-read guard in existing hook gates

Register gsd-secret-read-guard.js in every existing hook gate: the
hooks-crash-policy table (deny row; 6 -> 7 deny cases), plugin-manifest
REQUIRED_HOOKS and its Read|Grep|Bash group, docs-hooks-table-parity
EXPECTED_SURFACE_HOOKS, install.test MANAGED_JS_HOOKS, install-minimal-
hooks JS_HOOKS/BLOCKING_GUARDS, portable-node-runner GUARD_HOOKS,
kilo-upgrades PLUGIN_GUARD_HOOKS, the Kimi normalization-parity and
typed-payload floors, the OpenCode adapter (grep mapping, include ->
glob, three dispatch tests) and a Kimi TOML matcher assertion.

* fix(#4221): retire installer Read() deny rules (legacy filter)

Rename GSD_CLAUDE_DENY_PERMISSIONS to GSD_CLAUDE_LEGACY_DENY_PERMISSIONS
and stop adding the three Read(.env) / Read(.env.*) / Read(.secrets)
strings. mergeClaudePermissions now only filters them out of an existing
permissions.deny: an absent deny key stays absent, a malformed one is
still repaired to [], and an array emptied by the filter is deleted so
no `"deny": []` residue is left. Uninstall filters the same legacy list
and, symmetric with the Antigravity branch, drops an emptied allow or
deny key and an emptied permissions object.

Unlike the #2278 allow-side migration there is no surviving current
deny list, so the constant is renamed rather than mirrored. Removal is
byte-exact: a hand-written identical rule is indistinguishable from the
installer's and is removed too (the manifest never recorded permission
strings). USER-GUIDE and CONTEXT.md updated.

* test(#4221): flip install-regressions deny-rule assertions to the retired shape

The fresh-merge, non-destructive merge, idempotency, end-to-end install,
reinstall and uninstall assertions now expect no Read(.env*) deny rules
and no permissions.deny key on a fresh install; the deny:null repair case
is kept. A new describe block covers the legacy filter: retired strings
removed with a user entry kept, partial sets, near-miss strings
untouched, idempotency, GSD-only deny array deleted, a pre-existing
empty deny preserved, and uninstall symmetry for allow/deny/permissions.

Review round 1 (trek-e):

- Blocker: secret-name matching is now case-insensitive in the Read,
  Grep (path and glob) and Bash paths, so `.ENV` / `.Secrets` on a
  case-insensitive filesystem are recognized as the same secret file.
- Major: a shell interpreter's script is now scanned wherever it comes
  from. The tokenizer keeps heredoc bodies as per-segment tokens and
  records separator operators; pass 2 groups by segment id and resolves
  bash/sh/zsh/dash/ksh/su invocation mode: `-c` (including combined
  `-lc`) scans the script operand, a file operand is checked as a file
  (a `<( )` operand's echo/printf output is reconstructed), otherwise
  stdin is the script and heredocs, here-strings and a piped echo/printf
  source are scanned. `eval` joins all its operands; `source`/`.` handle
  process substitution. Data heredocs (`cat <<EOF`, the commit-message
  shape) stay unscanned.
- Major: `… | xargs <cmd>` checks the upstream segment's operands as
  file names when the sub-command reads (`echo .env | xargs cat`,
  `find . -name .env | xargs cat`); `-a`/`--arg-file` suppresses the
  inference; a shell sub-command's `-c` script is scanned.

Header, USER-GUIDE bullet and changeset updated; documented gaps now
include piped scripts from non-echo sources and `exec`/`timeout`
wrappers. 60 new suite cases pin the block and allow shapes.

* fix(#4087): stage the hook helpers the Codex bundle's hooks require

CODEX_HOOKS_TO_COPY is a flat, hand-maintained filename allowlist that never
recursed, and Codex is excluded from installSharedHooksBundle() — the path that
stages hooks/lib/ for full-bundle runtimes — by an !isCodex gate. Excluding
hooks/lib/ was a correct scoped decision for #3579 until #3911 (2ea5efc15) gave
gsd-context-monitor.js a real require('./lib/hook-exit.js'). From then on every
fresh --codex install staged the hook without its helper and the hook died with
MODULE_NOT_FOUND at module load, before its own try/catch, on every event Codex
registers it for. The install still exited 0, so nothing surfaced it.

Reproduced before changing anything, in a sandboxed CODEX_HOME: four hooks
staged, no lib/, and the installed hook exiting 1 on "Cannot find module
'./lib/hook-exit.js'".

Rather than hand-add today's three helpers — which re-breaks the next time a
Codex-bundled hook grows a lib dependency, exactly how this regressed — the
transitive-require walk already written for Cursor in 704859e9c is extracted out
of writeCursorHooksJson into an exported stageTransitiveHookLibs(), Cursor is
rewired onto it, and the Codex copy loop calls it. bin/install.js already
required that module, so this adds no new seam. Cursor's staged set is
byte-identical to base, compared file by file.

Extraction surfaced a latent defect in that walker, fixed here: its regex read
`./X` and `./lib/X` identically, but from a hook SCRIPT a bare `./X` is a
sibling in hooks/ — gsd-check-update-worker.js requires
`./managed-hooks-registry.cjs`, which is not a lib — so it demanded
hooks/lib/managed-hooks-registry.cjs and the fail-loud guard threw. Seeds now
match only `./lib/X`; lib files still match both, which is the
sibling-within-lib case 704859e9c exists for. Cursor never exposed it because
none of its scripts carries a bare sibling require.

Three further grammar gaps closed after review, each in the fail-closed
direction: an extensionless `require('./lib/x')` is valid CommonJS and was
resolved literally, failing the install on a legitimate require — now resolved
through .js/.cjs and written under its resolved name; a NESTED `./lib/sub/x.js`
could not be expressed by the character class and was a SILENT miss, the one
failure mode this function exists to remove — now refused loudly; and a capture
carrying no alphanumeric character is prose, not a module name — hooks/lib/
injection-patterns.js documents this very mechanism with the literal string
require('./lib/...'), which captured `...` and sent the resolver hunting for
hooks/lib/... . The scan is still not comment-aware, which is disclosed at the
call site rather than papered over.

Seeded from the entries THIS invocation staged rather than probing the
destination, so a file left by an earlier install whose source is no longer
allowlisted cannot contribute helpers for a hook that is no longer shipped.

The #3579 boundary holds: three of ten helpers ship, gsd-graphify-rebuild.sh
among those correctly absent. Seven rows — three driving a real install into a
sandboxed config dir (with HOME sandboxed for the child, since Codex's skills
kind resolves from os.homedir() and the #3712 guard rightly refuses otherwise)
and four pinning the discovery grammar directly. All proven fail-first; the
set-equality row also reds on over-staging, which the count-based version it
replaced did not catch.

Review of #4117, verified as asked and reproduced against a real install.

Windsurf sets hostBehaviors.skipSharedHooksInstall, so like Cursor it never
reaches installSharedHooksBundle -- the only other stager of hooks/lib --
and writeWindsurfHooksJson staged its two Cascade guards without the
helpers both require at module load: gsd-windsurf-pre-write.js requires
./lib/hook-exit.js and ./lib/git-probe.js, gsd-windsurf-pre-command.js
requires ./lib/hook-exit.js. stageTransitiveHookLibs had one call site,
Cursor's.

Measured on a fresh `--windsurf --global` install into a sandboxed HOME:
the installer exited 0, hooks/ held only the two scripts and package.json,
and executing either installed guard exited 1 with "Cannot find module
'./lib/hook-exit.js'" -- so every pre_write_code and pre_run_command event
failed at load while the install reported success. The same command with
`--cursor` staged four helpers and its hook ran, which is the control.

Pre-existing rather than introduced here: at merge-base 05092ff36 the same
three require lines exist and writeWindsurfHooksJson already staged no
lib/, and this PR's diff carried no reference to Windsurf. Fixed here
anyway because the helper this PR extracted is the right tool and a second
runtime is a few lines onto it.

writeWindsurfHooksJson now calls stageTransitiveHookLibs after staging its
scripts, with the same gsd: -> gsd- transform the scripts receive, so a
helper is rewritten the same way as its caller. The install-tree fixture
regenerates with exactly hook-exit.js, git-probe.js, cli-exit.js and
exit-code-registry.js added and no other fixture moved. Two rows execute
the INSTALLED guards, beside the Codex rows they mirror; the existing
windsurf-hooks-bridge rows run the guards from source and test behaviour,
a different question, and are left as they are. Both new rows fail-first
against the unfixed compiled artifact -- gsd-core/bin/lib, which is what
bin/install.js loads -- on the MODULE_NOT_FOUND assertion.

9eef1b791b fix(#3981): give blocking guards a host-stall-proof timeout budget (#4175)

Fresh registration must carry a host-stall-proof 120 s budget on the
six blocking PreToolUse guards; existing managed timeout:5 entries
are migrated; non-managed entries and advisory budgets are untouched.

Claude Code treats a timed-out hook as non-blocking, so the 5 s
budget on the six blocking PreToolUse guards silently dropped the
gates exactly when the host stalled under load. Registers them at
120 s (covers every observed stall, max 84.3 s) and migrates existing
managed timeout:5 entries in place, context-monitor-backfill shape.
Advisory hook budgets are unchanged.

107eb8c1d9 feat(#3753): run docs guards on the PR that changes the docs they read (#3787)
A PR whose diff is entirely under docs/ runs zero tests, so a guard whose INPUT
is shipped prose cannot protect the PR lane of the diffs it exists to check. Its
only firing opportunity is after merge, on the shared branch -- which is how next
went red on dacae9273 while the PR that caused it (#3746) was green on every
check.

The docs-lint job in .github/workflows/docs-required.yml -- an ALREADY-REQUIRED
context -- now selects and runs the docs guards that read the specific docs files
the PR changed.

  scripts/docs-guard-registry.cjs    test file -> the docs paths it reads (63)
  scripts/select-docs-guards.cjs     pure (changedPaths, registry) -> test files
  scripts/lint-docs-guard-registration.cjs   drift guard, wired into lint:ci

scripts/ci-test-scope.cjs is NOT touched -- `git diff origin/next --` on it is
empty -- so #764's saving stands and its 21 pinning tests are untouched.

Selection: exact path; trailing-slash directory prefix (boundary-checked --
docs/adrenaline.md does NOT match docs/adr/, which a naive startsWith gets
wrong); and '*' for the 6 entries that walk docs/ generally or read a computed
path. Unknown maps to '*' -- guessing narrow is how a guard silently stops
running. Measured: a typo fix selects 6 of 63; docs/AGENTS.md selects 12;
docs/COMMANDS.md selects 18.

Four things this got wrong first, each found by an independent reviewer or by
probe, and each having been asserted safe in a comment:

1. The registry started as a RULE in ci-test-scope.cjs's RULES, on the theory
   that classify()'s !codeChanged normalization made it inert. True for
   docs-ONLY diffs; false for MIXED docs+code diffs, where codeChanged is true
   and the normalization never runs:

     node scripts/ci-test-scope.cjs --files "docs/a.md src/semver.cts"
       with the RULE:  25 targeted_tests
       origin/next:     3 targeted_tests

   Category error: RULES is the scoped lane's input; a docs-guard registry is a
   lane manifest for a consumer that never calls classify(). Extracted; pinned
   by value.

2. The second attempt was a dedicated workflow with paths: [docs/**]. Such a
   workflow never reports on a non-docs PR, so it can never be a required
   context without hanging every non-docs PR -- and a non-required check does not
   block a merge, so the guard would have been advisory and #3753 unfixed.
   docs-required.yml already has no paths: filter, already supplies the required
   docs-lint context, already computes docs_changed, and already ran one docs
   guard gated on it. Generalizing that step needs no ruleset edit at all.

3. The registry and the drift lint were built from ONE path-segment heuristic, so
   both were blind identically -- and blind at the guard that motivated the issue.
   The reader-call regex required a character BEFORE its keyword, so a callee
   named exactly read( / load( / parse( / doc( / file( / content( could never
   match; and only an INLINE path.join(ROOT,'docs','X.md') argument was caught,
   missing the two-step-via-variable form -- the MAJORITY spelling -- plus
   template literals and concatenation. Detector 1 fired on 14 of ~450 files, so
   35 genuine guards sat unregistered while the lint reported 0 violations,
   including cursor-reviewer (reads docs/COMMANDS.md, asserts
   .includes('--cursor')) and inventory-headings-countfree. The "accepted blind
   spot" this shipped with was the common case, not a fringe.

4. With detection fixed the true population is 115 files: 63 genuine guards, 52
   incidental. Running all 63 in a REQUIRED check on a one-line typo fix is the
   cost #764 exists to avoid -- install.test.cjs is 7840 lines and reads exactly
   one docs file, docs/AGENTS.md, for its frontmatter. Dropping it reproduces the
   bug; running it for a typo elsewhere is waste. Hence the map.

Then a second review round found six more, all fixed here:

- fragment-single-edit-propagation.install.test.cjs was EXEMPTED as
  "overlay fixture only". False: it reads the real docs/registries/eos.json and
  asserts on a registry entry name, and reads the real ADR-0001 and asserts its
  H1. A docs-only PR touching either would have gone green and red next -- #3753
  shipping again, from inside the fix for it. Now registered against both paths,
  and all 52 remaining exemptions were re-audited one by one.
- The SUITES-collision guard compared RAW registry keys, but run-tests.cjs strips
  a leading `tests/` BEFORE its suite check. So it caught 'all' and missed
  'tests/all' -- the only spelling that can actually occur, since every key
  carries the prefix. One typo would have run all 824 test files inside the
  required job. Now normalized the same way run-tests.cjs normalizes.
- The lint failed OPEN on an unreadable tests dir or candidate file: 0 violations,
  ok:true. A guard that cannot read its input must never report success.
- The exemption ratchet gated identity only, so a baselined file that later
  STARTED asserting on shipped docs stayed exempt silently -- 52 permanently blind
  files. The baseline now fingerprints the docs paths each exempted file
  references and fails when that set changes, naming what changed.
- The exemption marker was still honored inside a multi-line template literal in
  the header window. The scanner now tracks template-literal and block-comment
  state.
- `git diff --name-only | grep '^docs/'` silently dropped C-quoted non-ASCII docs
  paths, making docs_changed=false a green zero-guard check. Both call sites now
  pass -c core.quotepath=false.
- The run step was gated on hashFiles(), which a force-committed
  .docs-guard-tests.txt would satisfy. The step now rm -f's both scratch files
  first and gates on an output it sets itself.

Three empty states, deliberately distinct, because conflating them rebuilds
#3753: an empty or malformed registry HARD-FAILS; docs changed with no guard
covering them logs and skips; no docs change is already gated. The middle state
must never be expressed as an empty --files-from, which prints `no tests in suite
"all"` and exits 0 -- a green check that guarded nothing. With the current
registry that state is unreachable, because the six '*' entries always match;
the branch is kept as defensive handling for a future registry and says so.

timeout-minutes: 15 bounds the required job against a hanging fork-supplied test;
it had none. npm ci was added because the job never installed dependencies -- the
previous single-file step got away without it, the registry does not.

docs/contributing/docs-guard-registration.md documents the rule, following its
sibling cross-platform-portability-rules.md, and CONTRIBUTING.md's CI Test
Quality Checks table links to it. It is also load-bearing: without a docs/ file
in the diff this PR would not have triggered its own lane, shipping an
unexercised change to a required check.

One unrelated fix, included because this PR surfaced it and CLAUDE.md forbids
deferring a defect found while working. On this branch's first CI run,
`full test (windows-latest, 24, shard 3/3)` was CANCELLED at exactly 30 minutes;
tests were still passing 0.8s before the cancel, so it is a wall-clock timeout,
not a hang, and a cancelled job reddens `Required tests`.

The cause is not this PR's test file, which costs ~60ms. Shard composition is
unstable: adding ONE file to the unit suite reshuffled 115 of 268 files between
shards, and shard 3 drew a heavier mix. Underneath that is a real pre-existing
defect. tests/ci-test-job-timeout-budget.test.cjs requires every lane's budget to
be >= 1.5x its MEASURED cost -- "a lane that got slower must be re-budgeted, not
excused" -- and its test-full entry recorded 19m from a windows-22 shard. That is
stale. Measured on `next` with none of this PR's changes present: 26m18s (run
32614439702, windows-latest/24 shard 3/3), 23m36s and 23m17s on shard 2/3. So the
lane costs ~26m and the 30-minute cap carried 1.14x headroom, not 1.5x. The gate
had been out of compliance with its own rule; this PR was merely the file
addition that reshuffled shard 3 past the cliff.

Fixed as that file prescribes: measuredMinutes 19 -> 27 with fresh evidence, and
test-full timeout-minutes 30 -> 45. The rule's minimum for 27m is 41; 45 is
deliberately above it because the reshuffle means per-shard worst case moves run
to run, and a budget pinned to the exact minimum would be re-breached by the next
test file anyone adds. Only that one job's timeout changed; test.yml's scope,
matrix and steps are untouched, so #764's saving is unaffected.

Raising that cap let the Windows shard finish (28m45s, inside 45) and uncovered
a real failure the 30-minute cancel had been masking:
`new quick-task branch branches off origin/main (#2916)` died with
`outcome=timed_out exitCode=null`, SIGTERM, at the 15000ms bound.

tests/quick-branching.test.cjs:149 `runStep` runs a `#!/usr/bin/env bash` script
executing MULTIPLE git commands, but was bound to GIT_TIMEOUT_MS (15000) -- the
norm for a SINGLE git plumbing call. tests/helpers/timeouts.cjs already documents
this exact failure and exists to fix it: HOOK_FANOUT_TIMEOUT_MS was created after
PR #3285 recorded "outcome=timed_out exitCode=null at exactly the 15000ms probe
bound while every other lane passed the same commit", and calls that "a bound
sized for the wrong class, not a slow machine". Our failure is that case
verbatim, so both sites move to the class norm rather than to a bigger number.

The same class also failed on `next` itself 21 hours earlier -- run 32608945654,
windows-latest/24 shard 1/3, `plan touching only src/ in a submodule project
keeps worktree isolation ENABLED` -- where tests/worktree-safety.test.cjs:5845
`runGate` fans out to `git config --file .gitmodules` under a hardcoded 30000.
Fixed too, since it is a defect in the tree regardless of which branch surfaced
it.

A survey of the whole tests/ tree found the same class-mismatch at further
bash fan-out sites bound under 60000ms, and the maintainer approved sweeping
them rather than leaving them latent to surface the same way one at a time. 16
fan-out sites across 16 files now use the class norm.

The sweep is class-correctness, not raising numbers until things pass. Sites
were moved ONLY where the bash body demonstrably spawns something (git, node,
npm, a CLI); self-contained shell snippets were left where they are, and are
listed as deliberately unchanged: pure if/printf bodies (copilot-install), pure
array/case builtins (code-review-pipeline-regression:638), a documented
pure-shell gsd_run stub (host-integration), single-process hook calls
(workflow-guard:222/271/302), and a deliberately tight 5000ms fast-check hook
(gsd-write-guard.property). Nothing was lowered. process-seam.test.cjs:513
(literal 300) is untouched on purpose -- it tests timeout BEHAVIOR, so raising
it would destroy what it asserts.

Shared file-level constants were the trap here, and were handled per file rather
than by redefinition: GIT_TIMEOUT_MS has ~15 users in git-base-branch and only 1
is a fan-out; WORKTREE_TIMEOUT_MS has 16 users in worktree.test.cjs and 3 are;
PROBE_TIMEOUT_MS has several in three more files. In each the CALL SITE was
changed and the constant left alone, so no single-plumbing-call site silently
inherited a 60s bound. The one exception is hooks-opt-in.test.cjs, where
HOOK_TIMEOUT_MS has exactly one consumer -- spawnHook, the fan-out itself -- so
redefining it is identical in effect and reads better.

Only two of these sites have actually been observed failing. The rest cite that
shared class and those two run ids rather than inventing evidence of their own.

The installer exported 197 names and had zero production consumers - every
non-test require of it repo-wide sits inside a comment. Its interface was
shaped by test access, not by callers.

Removes 9 dead exports and 61 pass-throughs, repointing their tests onto the
extracted modules' own interfaces. 197 down to 127.

Every count in the issue was wrong: 197 exports not 188, 9 dead not 12, 61
pass-throughs not 49, 44 test files not 42 - and the audit itself then missed
7 more consumer files. restoreUserArtifacts was on the dead list but ceased to
exist in phase 6, and two _GSD_EFFORT_MANIFEST_* names listed as dead are now
genuinely asserted, so acting on that list would have deleted live exports.

7 of the 9 dead names collide with an independent declaration that install.js
delegates TO. Each removal was justified by which declaration a reference
resolves to, never by whether the name appears somewhere.

Coverage parity was the gate rather than test greenness: per-file counts were
captured before any edit and diffed after. 44 of 45 files are byte-identical;
the single delta is one added assertion, not a loss.

The sweep for scattered require sites found two forms static grep misses -
require(VARIABLE) and multi-line require() - plus tests asserting that
install.js re-exports the SAME object, which now assert retirement instead.

Both review engines found real defects in the first cut.

The DEFECT.GENERATIVE-FIX single-owner guard from #1511 had been repointed
from a reference-identity check to install.X === undefined. Those are not
equivalent: the guard exists to catch a duplicate function body reintroduced
into install.js, and the replacement passes cleanly if that duplicate is used
internally and never exported. It now walks bin/install.js's real top-level
bindings, so it catches a duplicate under either shape, exported or not -
strictly stronger than the check it replaced. Proved by injecting a duplicate
and watching it go red.

That weakening survived the coverage-parity gate because the assertion count
never moved. The gate compares counts, so an assertion that changes meaning
rather than number is invisible to it.

Removing the exports had orphaned their wrapper bodies: 14 dead wrappers, 9
consts and 9 destructure entries, several pre-existing and found by the same
sweep. Dead code left in the file this phase exists to shrink.

Three more comments claimed re-exports this phase removed, and tests were
reading Cursor and Windsurf hook constants from install.js's local copy while
calling functions from the hooks surface - equal today, with nothing holding
them equal. The local consts now reference the owning module.

c66b010052 chore(#3508): site-scoped allow-test-rule suppression (#3512)
Phase 4 of #3464, following #3465, #3466 and #3502. Those cut the ceiling
305 -> 278 and made the rule accurate. This closes the remaining structural
weakness: suppression was FILE-WIDE, so a single justified exemption silently
absolved every other source-grep in that file, forever, including ones added
later by someone else.

hasAllowAnnotation did comments.some(...) over the whole file and returned {}
early. A marker is now checked per report: a violation is suppressed only by a
marker on its own line, or on a line above it with nothing but blank lines and
other comments in between, bounded by MAX_MARKER_LOOKAHEAD_LINES = 8.

The bound is comment-purity rather than raw distance, and that distinction is
load-bearing: an intervening line of real code (a `test(...)` opener, say)
ends the window even when the marker is physically close. Chosen from the
actual placements in the affected files rather than picked a priori -- the
repo's convention puts several lines of prose rationale between the marker and
the code, so a tighter rule would have invalidated legitimate existing markers
and forced churn for no correctness gain.

Measured before writing any code, by running the real rule with the
suppression check neutralized across all 1194 files its globs match: 14
violation sites in 8 files, and ZERO in files carrying no marker -- so the
green build was legitimate, and the entire migration surface was those 14.

11 sites were mechanical: an existing marker already stated the right reason,
it just sat too far away. Those were relocated to their call sites with the
original #NNN citations preserved.

Three were orphans -- the file's markers were about an entirely different
concern and nobody had ever justified these reads. All three are fixed
BEHAVIORALLY, with no new markers:

  install-minimal-hooks.test.cjs:975 asserted
  src.includes('gsd-update-check') && src.includes('replace(') against
  bin/install.js. It now calls the exported stripStaleGsdHookBlocks() on a
  legacy TOML fixture and asserts the actual stripped output. This is the
  case this phase was opened around: it could be added with no review friction
  and stay invisible indefinitely under file-wide amnesty.

  config.test.cjs:1917 regex-tested src/init.cts for detectGitCreateTag. It now
  drives `init complete-milestone` and asserts the git_create_tag field.

  config-schema.property.test.cjs:1107 did the same for detectFallowConfig; it
  now drives `init code-review` and asserts fallow_enabled.

Each was proven RED against a broken production file and GREEN against the real
one, with src/init.cts and bin/install.js confirmed byte-identical afterwards.

Marker lines in the 8 files went 20 -> 24, against a filed expectation of
"must not increase" (projected 14). That projection was wrong and is corrected
on #3508 rather than met by deletion. It assumed every existing marker was a
distant blanket that site-scoping would consolidate. Some are already
site-adjacent and guard real source-greps the rule CANNOT detect -- verified in
install-minimal-hooks.test.cjs:2686-2757, where seven markers each sit directly
above a readFileSync(reloadScript) + .includes() pair reading
hooks/gsd-config-reload.js. Removing them to hit a number would have repeated
the Phase 1 mistake: deleting markers on "the rule doesn't fire" evidence when
the rule provably cannot see the violation.

An earlier revision of this commit message attributed that invisibility to the
#3502 dynamic-path blind spot, on the grounds that reloadScript is a variable.
Adversarial review caught that as a false causal claim and it is corrected
here. looksLikeSourcePath's hasSourceDir regex is
/['"](?:bin|lib|gsd-core|src)['"]/i, and those reads target hooks/ -- so a
fully literal path.join(ROOT,'hooks','gsd-config-reload.js') is equally
invisible. The variable indirection is irrelevant. This is a FIFTH, distinct
blind spot: the source-dir allowlist omits hooks/, which is a real shipped
production directory (eslint.config.mjs registers its own rule block for
hooks/**/*.js). Recorded in 40-design.md Known limits and left for a follow-on
phase -- widening the allowlist is unmeasured, and measuring before widening is
the discipline #3502 established. The conclusion was right; the stated
mechanism was not, and asserting an unverified cause is the error being
corrected.

The honest metric is not fewer markers. It is that every marker now sits
adjacent to the specific read it justifies instead of absolving a whole file.
Site-scoping turns one blanket marker covering N sites into N site markers by
design; the count rising is the mechanism working.

A second review finding is fixed here too. Suppression originally keyed only
off the text-search line, so a marker placed directly above the readFileSync()
call -- the intuitive place to annotate "this read is fine" -- did NOT suppress
when the search sat on the following line, because the read's own assignment
line breaks comment-purity. It failed safe (a loud error, never silent
suppression), but it was a trap contributors would hit, and it contradicted
this change's own claim that the placement rule would not force churn. A
violation is now suppressed by a marker adjacent to EITHER the search site or
the originating read. The violation is fundamentally the read+search pair, so
annotating either half is legitimate, and it stays strictly site-scoped -- the
decisive isolation row still holds.

17 RuleTester rows cover the new semantics. The decisive one asserts that a
marker adjacent to one violation does NOT suppress an unrelated violation
elsewhere in the same file -- exactly 1 error, reported at the second site.
Teeth-checked by reverting the predicate to file-wide, confirming that row and
two others flip pass->fail, then restoring. Two pre-existing RuleTester cases
that asserted the old file-wide semantics were corrected.

Compatibility held where it matters: 277 marker-bearing files have no
detectable violation at all, and site-scoping makes their markers no-ops rather
than errors. All stay green, untouched.

Ceiling unchanged at 278; lint-allow-test-rule-refs reports 278/278.

Deliberately not done here, and recorded for the follow-on phase: the same
measurement found only 8 of 285 marker-bearing files contain a detectable
violation. That suggests a large honest ceiling drop, but "the rule doesn't
fire" is the unsound oracle that forced the Phase 1 revert of 295 files, and
the rule still has documented blind spots -- as install-minimal-hooks itself
demonstrates above. It needs two independent signals agreeing, which is only
credible now that the rule is accurate. With suppression site-scoped, "an
effective exemption" is finally well-defined, which is what makes re-pointing
the ratchet at effective exemptions -- rather than at marker-text presence --
the natural next step.

8a6d87538f test(#3466): replace 8 source-grep assertions with behavioral tests (#3500)
Phase 2 of #3464, following #3465. Rewrites every assertion that read a
shipped .cjs/.js file and text-searched it, so the file no longer needs an
allow-test-rule exemption. 6 of the 8 files are now marker-free; the ceiling
drops 285 -> 278 against a measured 277.

A source-grep passes when a STRING is present, not when the code WORKS. It
survives a refactor that keeps the string but breaks the behavior, and breaks
on a refactor that keeps the behavior but renames the string. Both failure
modes are silent about the thing the test claims to protect. That is the
anti-pattern ADR-456 and local/no-source-grep exist to prevent.

Rewrites, each against the real exported seam:

- discuss-mode: calls cmdInitPlanPhase() against a fixture whose config sets
  workflow.text_mode, asserts the value propagates to its emitted JSON.
- effort-surface-axis: runs review-lane invoke against a real project with a
  fake `claude` PATH shim, asserts the resolved --effort actually lands in the
  shim's captured argv.
- install-minimal-hooks: calls applySettingsJsonHooks() with a hook source
  missing, asserts it is neither registered nor silently registers wholesale,
  with sibling present hooks as the positive control.
- install: calls the exported inferPreferredRuntime({fs, env,
  preferredConfigDir}) via its injected fs seam, asserting 'kilo' from both
  the config-marker and env-var paths.
- opencode-permissions: spawns the real installer with a custom config dir,
  asserts the written opencode.json permission paths are anchored on it.
- repo-layout: spawns the installer for copilot local vs global, asserting
  AGENTS.md is written only in the local case.
- runtime-config-adapter-registry: stubs resolveInstallPlan and force-reloads
  install.js, asserting the runtime's artifact stops being written -- proving
  install.js genuinely routes through the registry.
- runtime-homes-descriptor-drive: calls buildAgentSkillsBlock() for cursor and
  claude against real fixture SKILL.md files, asserting each runtime's refs
  land under its own config dir and never the other's.

Every rewrite was mutation-checked before its marker was dropped. Because
node --test is not runnable locally in this repo, each assertion was
replicated in a standalone probe that requires the same module: run green
against the real file, then red against a deliberately broken one (text_mode
propagation deleted, existsSync guard removed, config dir hardcoded, !isGlobal
guard dropped, kilo branch removed, effort resolution bypassed, skills base
hardcoded back to .claude), then the production file restored and confirmed
byte-identical. An assertion that could not be made to fail would not have
shipped -- a behavioral test that passes regardless of correctness is strictly
worse than the source-grep it replaces, because it looks rigorous while
asserting nothing.

Three claims were checked rather than trusted. All three were wrong:

- repo-layout's own marker cited #1188 asserting the `!isGlobal` lexical scope
  was "unprovable at runtime". It is provable: the guard decides whether
  AGENTS.md is written, which is directly observable. Both directions verified.
- The triage for runtime-config-adapter-registry claimed its source-grep was
  redundant with the file's EXPECTED_TABLE tests, so deletion would be safe.
  Those tests only exercise resolveInstallPlan() directly and never load
  bin/install.js, so they do not cover it. A real behavioral assertion was
  written instead of deleting coverage.
- An earlier revision of this change dropped runtime-config-adapter-registry's
  two markers on the grounds that ESLint stayed silent without them. An
  adversarial review caught that this was wrong, and it is restored here. The
  file still genuinely source-greps bin/install.js at two sites; ESLint is
  silent only because no-source-grep's TEXT_METHODS omits matchAll. Dropping a
  marker because the linter cannot see the violation is exploiting the blind
  spot, not resolving it -- and it would go red the moment the rule is
  widened. Those two assertions are also irreducible: they assert that EVERY
  inline `runtime === '...'` branch in install.js names a registry-known
  runtime, and a branch naming an unregistered runtime would simply never
  execute, so no runtime observation can prove its absence. The markers now
  say so explicitly.

Two coverage gaps in no-source-grep surfaced while doing this, recorded on
#3464 rather than fixed here, since widening the rule is its own change with
its own blast radius:

- TEXT_METHODS omits matchAll, so a matchAll source-grep never trips the rule
  (the case above).
- The path test requires a literal quoted bin/lib/gsd-core/src segment and
  tracks the binding one hop, so a read through a dynamic path or an
  intermediate variable evades it. install-minimal-hooks' genuinely
  load-bearing read at line 975 is itself unmarked for a related reason.

install-minimal-hooks therefore keeps its markers too: its remaining real
source-grep of bin/install.js (the Codex legacy gsd-update-check migration
check, line 975) is outside this issue's 8 sites. It is the last blocker for
that file and is a clean follow-up.

c28134ab39 fix(#3271): delete 25 duplicated folded test suites and fix three runner defects found doing it (#3285)
* test(#3271): guard against a folded suite appearing twice in one host

Adds local/no-duplicate-fold-marker, an AST rule that reports the second and
every subsequent `folded:<name>` marker in a host file, plus RuleTester cases
and a tree-wide regression assertion.

Failing-first on purpose: the rule is registered at error and the 25 duplicated
regions are still present, so eslint and the new tree-wide test are RED. The
deletions land in the next commit.

The marker key is the whitespace-delimited token after `folded:` — not the
issue's `[a-z0-9-]*` slice, which truncates at `.` and false-positives on
tests/model-resolver.test.cjs where feat-443-effort-fast-mode.integration and
feat-443-effort-fast-mode are two distinct folded suites.

Three consolidated install suites each carried a verbatim second copy of a
contiguous run of #1969 B1 folded blocks. Byte-identical, constant offset, and
green — each duplicated block registered and ran twice on every lane.

Introduced by 6d072435d (#1975 re-applying #1970's hunks on a tree that already
had them, 2026-07-03) — one stale-base re-application, three files, one commit.
Verified by marker-count bisect: 1 at 4f779eda4 and 0cc7a1a42, 2 from 6d072435d
onward.

The later copy is deleted in each case, so every file returns to what its
authoring batch produced and blame on the surviving lines stays accurate.
local/no-duplicate-fold-marker, red on the previous commit, is now green.

tests/model-resolver.test.cjs is untouched: the issue lists it, but its two
blocks are folded from two different files and are not identical. It is a false
positive of the issue's own grep, whose `[a-z0-9-]*` key truncates at `.`.

Fixes #3271

* test(#3271): property-test marker identity and pin the alias non-goal

Three review findings, all fixed inline:

1. foldMarkerOf is a parser and carried no fast-check property test. Raised
   independently by the /code-review standards axis and the isolated adversarial
   pass; the file already establishes the fc.property-driving-ruleTester idiom
   for a sibling rule. Added, two arms over markers generated from [a-z0-9-._]:
   the same marker twice always reports exactly once against firstLine 1, and
   two distinct markers never collide. The alphabet includes `.` on purpose —
   an implementation keyed on the issue's [a-z0-9-]* slice passes arm 1 and
   fails arm 2, which is exactly the model-resolver false positive.

2. meta.docs.category was the novel value 'Test hygiene'; all 16 sibling local
   rules use 'Best Practices', 'Portability' or 'Reliability'. Now
   'Best Practices'.

3. A call through a further alias (const d = __foldDescribe) was unreported and
   undocumented — accidental rather than deliberate. It is now the fourth entry
   in the rule's documented non-goals, with the reason, and pinned by a valid
   RuleTester case so it cannot drift silently.

* test(#3271): name the step and elapsed time when a baseline build fails

buildBaselineAtRef runs four bounded steps and, when one exceeded its bound,
threw a bare "spawnSync ETIMEDOUT" naming neither the step nor how long
anything took. Diagnosing one real failure took four separate experiments to
recover information the throw already had.

Each step is now timed, and any throw carries the breakdown: which step failed,
its elapsed time, the timings of every step that completed before it, all three
bounds, and the tail of the child's captured stdout/stderr.

The failure message is deliberately the carrier. On the remote runner the
captured output field comes back empty in failures.json while error and stack
survive verbatim, so the message is the only channel that reaches a reader of a
remote verdict.

* fix(#3271): size the baseline generator bound for the machine it runs on

Instrumentation from a real remote-runner failure gave the breakdown:

  git-worktree-add=15.1s  npm-run-build-lib=19.8s  gen-emitted-baseline=FAILED@300.1s

Steps 1 and 2 are comfortable. Only the generator exceeds its bound, and it is
not hung — it needs more than 300s there.

Measured ladder for that step: ~22s idle in a container, ~39s end-to-end in a
clean container, ~142s with 8 CPU burners on 8 cores, and >300s under the real
suite. Its cost is 19 sequential installer spawns, and spawn latency is exactly
where a container degrades worst (3.9x slower than host, against 1.1x for file
IO) — which is why a CPU-only load test did not reproduce it and why four
earlier hypotheses (container slowness, network, shallow clone, CPU contention)
all measured clean.

The 300s bound was sized on an idle machine for a step that never runs on one.
Under the remote runner the on-disk baseline cache is structurally absent — CI
restores it via actions/cache keyed on github.event.pull_request.base.sha, a key
that exists only inside GitHub Actions — so this slow path runs on every remote
verification. The result: this gate has passed 0 times in 754 runs, failing 80
times and never once executing successfully.

Raised to the 600000ms ceiling that local/no-unbounded-spawn treats as the
largest meaningful bound; the other two bounds are untouched. This makes the
gate RUN, which is the point: the alternative considered and rejected was
degrading the timeout to a skip, and that was measured to turn the suite green
with the gate silently not running at all.

The real remedy is making the cache reachable from the remote runner so the
in-job build returns to being the rare fallback ADR-2719 §5 describes. That is a
gsd-test-runner change, not one this repo can make.

* fix(#3271): tolerate an overlay source that vanishes mid-walk

Observed on the remote runner, three runs across three different branches:

  ENOENT: no such file or directory, link '/work/hooks/dist/gsd-config-reload.js'
    -> '/tmp/gsd-2930-overlay-6nOZay/hooks/dist/gsd-config-reload.js'

buildOverlayRepo enumerates names with readdirSync and then acts on each one, so
statSync, copyFileSync and linkSync all sit in a TOCTOU window. hooks/dist is
regenerated by an ATOMIC REPLACE (scripts/build-hooks.js unlinks and renames), so
any concurrently running test that rebuilds hooks retires a just-listed name
mid-walk and the overlay dies on it. linkOrCopyFile already tolerated EXDEV and
EPERM; ENOENT went straight through.

On ENOENT the source is now re-examined ONCE rather than slept on. An atomic
rename is a single syscall, so by the time the failure surfaces the successor is
either already in place (the retry succeeds) or the path has genuinely left the
tree, in which case there is nothing to mirror and the leaf is skipped. No sleep
and no spin: a timing-based wait here would be the very flake being fixed. Every
other errno still propagates untouched, so a real permission or IO fault stays a
hard failure.

Five tests hold the boundary: gone-for-good skips without retrying, mid-replace
retries exactly once and places the file, EACCES still throws, a real linkSync
ENOENT is injected by monkeypatching fs and restoring it in a finally (never a
mode-bit trick, which root bypasses), and isMissingPath accepts only ENOENT.

* fix(#3271): order the timeout ladder inward-out and lock it

Two review blockers, both real.

The generator bound had been raised to 600000ms — exactly the whole-chunk timeout
in scripts/run-tests.cjs:973. A step bound equal to the chunk ceiling loses the
race: the chunk is killed first and the failure arrives as an opaque "no failed
step" kill, so the per-step diagnostic added a commit earlier was built and then
made unreachable in the same change.

Separately the #2767 test declared a per-test timeout of 300000ms, BELOW the
inner bound it was meant to permit, so it could still die at the exact 300s
ceiling this was supposed to lift — via node:test's timeout rather than
spawnSync's. Its sibling declared 900000ms, above the chunk ceiling, which is the
same opaque-kill hazard from the other direction.

The three bounds only produce a useful failure if they fire inward-out, so they
now do: step 360s, per-test 480s, chunk 600s. 360s is ~3x the passing observation
(91.6s / 115.8s) and 20% above the censored 300.1s timeout, while leaving 240s of
chunk headroom for every other file sharing it. Four tests lock the ordering,
including a drift guard on the exported values — without it, editing a call
site's literal timeout would leave the ordering assertions passing while the real
ladder inverted.

Also from review:

- err.gsdBaselineStep and err.gsdBaselineTimings were written and never read
  anywhere in the tree; only the rewritten message is consumed. Removed rather
  than kept as speculative surface.
- buildOverlayRepo discarded placeVanishableLeaf's boolean at both call sites, so
  a vanished leaf left the overlay with no accounting at all. It now collects the
  skipped paths and warns once. Not thrown: a source that left the tree really is
  not part of the snapshot, and throwing would reintroduce the crash the
  tolerance removes — but silence would let a dropped leaf resurface later as an
  unrelated missing-file assertion.
- The instrumentation commit shipped no test. One now drives a real failure and
  asserts the message names the step, its elapsed time, and the bounds.

CI failure on PR #3285, job full test (windows-latest, 22, shard 2/3) — every
other lane green, including windows-latest node 24 across all three shards:

  not ok 1 - blocks push when any to-be-pushed commit matches local blocked regex
    error: bash .githooks\pre-push failed — outcome=timed_out exitCode=null stderr=
    duration_ms: 15040.2168

A bound, not a hang: the test supplies stdin via input:, so the hook is not
blocked reading its ref list, and the duration lands exactly on the 15000ms
bound.

The site used PROBE_TIMEOUT_MS, which tests/helpers/timeouts.cjs documents as "a
single short CLI query or node -e probe against a temp fixture". This is not
that. It spawns bash running .githooks/pre-push, and the hook then invokes a MOCK
git that is itself a bash script, so one runHook is roughly four Git Bash spawns.
On Windows each is Defender-scanned and the first hook test in a file pays cold
start on top. That module's own docstring warns against precisely this: a call
site that differs from its class must not be forced onto a shared value that does
not describe it.

HOOK_FANOUT_TIMEOUT_MS is that missing class — 60000ms, 4x the bound that failed
and half INSTALL_TIMEOUT_MS, which is the right order: a hook fan-out is much
lighter than a full installer run and far heavier than reading back a version
string. Two tests lock the ordering against both neighbours, including one
asserting real margin over the censored 15040ms observation, since a bound that
merely matched what was measured would be the same defect again.

Scoped deliberately: the other ~360 runHook sites keep their current bounds. This
adds the norm and applies it where a real failure demonstrated the need, rather
than sweeping a value across sites with no evidence for any of them.

* test(#2704): failing-first coverage for ADR link resolution and H1 status brackets

Binds the gate to two assertions it does not yet make: every relative markdown
link under docs/adr/ must resolve, and an H1 trailing status bracket must agree
with the Status: field instead of being silently stripped.

Covers all 51 rows of the phase test matrix across two altitudes - the pure
extractLinks/maskCode IR for fence and inline-code-span boundaries, hostile
input and the fast-check totality properties, and the real CLI verdict for the
end-to-end classes. Includes the DEFECT.GENERATIVE-FIX parity test that iterates
the exported STATUSES array so a sixth status is covered the day it is added.

* feat(#2704): resolve ADR documentation links and compare H1 status brackets

The ADR gate validated naming, relation symmetry and index freshness but never
resolved a link target, and it stripped an ADR's trailing H1 status bracket for
display rather than comparing it against that ADR's own Status: field. Both
classes were structurally invisible: #2691 found five dangling references by
manual audit roughly a year after they were introduced, one of which reached the
published npm payload, while CI reported green throughout.

Both are now assertions on the same --check path, using only node:fs and
node:path - no dependency and no subprocess.

Fenced blocks and inline code spans are masked before scanning, because markdown
does not render a link inside code. That is not a policy choice: the corpus
contains exactly two such sequences today and both are ordinary JavaScript.
Masking preserves length and column positions so findings still name a real line.

Resolution is case-exact on every platform - a link that resolves only through
macOS or Windows case-folding still 404s on github.com and still fails the Linux
lane - and a destination resolving outside the repository is reported before any
filesystem call is made.

Also single-sources two duplicated surfaces this change would otherwise have
extended: the H1 bracket vocabulary (a second hand-written copy of STATUSES with
nothing asserting agreement, a DEFECT.GENERATIVE-FIX instance) and the docs/adr
directory traversal. Two tests added by #2691 that reimplemented link resolution
and bracket comparison inside the test file are removed for the same reason; the
corpus assertion is now made by running the real gate against the real corpus.

* fix(#2704): reject symlinks that leave the repository and linearize code masking

Four defects from the isolated adversarial security review, plus one it noted.

BLOCKER - a symlink defeated path containment. path.relative(ROOT, abs) is
purely lexical, but the case-exact walk then calls readdirSync, which follows
symlinks at the OS level: a contributor-committed docs/adr/x -> /etc together
with a link through it passed containment and listed the real external
directory, and a wrong-case probe echoed a real external filename through the
"Did you mean" hint into publicly-readable fork-PR logs. Every segment is now
lstat'd before descent; a symlink is realpathed and re-checked against
realpath(ROOT) - realpath on both sides, so a root under /var does not produce
false escapes - and an escape emits no hint and reads nothing further.

The same rule now governs which FILES are read: an ADR entry that is a symlink
out of the repository is excluded and reported rather than parsed, closing the
vector this change had widened by newly reading README.md, naming-violation
files, and full bodies rather than only header fields.

MAJOR - inline-span masking rescanned the line remainder per backtick run,
roughly O(n^1.6) on adversarial input: 1.76s for an 800KB line. Rewritten as a
single linear pass pairing runs through forward-only per-length cursors. Same
input now takes 3.31ms, with behavior unchanged.

MINOR - an unreadable or broken entry threw, and the generic handler wrote a
raw stack trace carrying absolute CI paths to stderr. The scan is now
fault-tolerant and reports excluded entries as ordinary violations. The status
vocabulary is escaped before being interpolated into a dynamic RegExp -
defence-in-depth, not a live bug.

The containment predicate had reached three hand-written copies while fixing
this; it is now the single escapesRoot() helper used by all four call sites.

* feat(#2704): add a --json report so the gate's tests assert on typed values

Maintainer-directed addition. CONTRIBUTING.md's "Prohibited: Raw Text Matching
on Test Outputs" requires that a system under test producing text also expose a
structured intermediate representation, and that tests assert on that IR rather
than on rendered prose. This gate had no such surface, so its verdict tests
matched on stderr.

--json runs exactly the same validation as --check and writes a report to stdout
with the same exit code, following the frozen-REASON-enum pattern already used
by verify-reapply-patches.cjs. Every violation carries a stable reason code plus
the fields a consumer needs, so nothing has to pattern-match an error message.
Adding a reason stays three coordinated changes - the enum, the emitting site,
and the test locking Object.keys(REASON).sort().

The human output is unchanged, deliberately: a large pre-existing suite asserts
on it and migrating that is not this PR's concern. Verified by running the
pre-change and post-change scripts against an identical violating corpus and
diffing their stderr - character-for-character identical.

This PR's own verdict tests now assert on parsed --json. Absence checks improve
the most: "no bracket violation" is now a reason-code predicate rather than a
negative regex over prose, which could pass for the wrong reason. The security
assertions were strengthened rather than translated - no leaked filename may
appear in ANY field of the serialized report.

Unknown flags are now rejected instead of silently falling through to printing
the index.

* test(#2704): fix the status-parity fixture and guard hooks/dist before overlay builds

Two failures from the matrix run of 79b29909.

The status-parity fixture was mine. It built, per status token, an ADR whose H1
bracket and Status field both carried that token - but Superseded carries an
obligation beyond the bracket: it must name its successor as a file link and be
symmetric with it. The fixture declared a bare Superseded, tripped that
unrelated invariant, and the test reported a bracket-parity failure for a reason
that had nothing to do with bracket parity. The fixture now satisfies each
token's own obligations in both the agreeing and contradicting corpora, derived
from the status actually declared rather than special-cased on one name, so a
future token carrying obligations is handled rather than silently skipped.

The second failure was not mine but is fixed here rather than deferred.
mcp-catalog-parity.install.test.cjs hardlinks hooks/dist/* while building its
overlay, but hooks/dist is a gitignored build artifact produced only by
build:hooks. The suite had no guard, so it passed only when some other suite
happened to build it first - an execution-order dependency, which is why it
failed on node22 and passed on node24 for identical code. install.test.cjs
already documents this exact hazard and guards it.

Six behaviorally identical copies of that guard existed across three files.
Rather than add a seventh, they are now one canonical
tests/helpers/hooks-dist.cjs - idempotent and bounded by the shared
BUILD_TIMEOUT_MS class norm - which is the same single-sourcing this PR applies
to the ADR gate itself.

* docs(#2704): add a how-to for contributors the ADR gate rejects

The reference and explanation quadrants were covered by Lifecycle rules 5 and 6,
but the task-oriented one was thin: a contributor meets this gate because it
failed on their PR, under pressure, and the rules told them what is checked
without telling them what to do about it.

Adds the command to reproduce the CI failure locally and a message-to-remedy
table covering every reason code that can be hit - unresolved target, wrong case
with the did-you-mean hint, repository escape, symlinked ADR file, bracket
contradiction - plus the backtick escape hatch for illustrative links and the
caveat that indented code blocks are not skipped.

The table is itself written in backticked inline code, so the gate skips it: the
escape hatch demonstrated on the page that documents it.

* test(#3145): bound the installer/runtime cluster onto the process seam

Migrates 156 unbounded sync spawn sites across 47 files. Allowlist 120 to 73.

Timeouts are sized from evidence already in the tree rather than a house
default, because this wave spawns installers rather than git plumbing and an
undersized bound does not catch a hang -- it manufactures CI flake, which is
worse, since a flake gets re-run instead of investigated. install.test.cjs
records a real spawnSync ETIMEDOUT at a 60000ms cap on a loaded bench while
another lane passed the same commit in 12.7s, so full installs are bound at
120000ms against that recorded incident.

Also adds an auditable escape to the guard's timeout ceiling. The 600000ms
cap was set in #3143 from partial evidence, but fragment-single-edit-
propagation carries a documented, load-tested 900000ms bound on a run that
chains a full build plus eight generators -- the guard would have rejected a
correct timeout the moment that file left the allowlist. A value above the
ceiling is now permitted only with an inline allow-spawn-timeout-ceiling
marker carrying a non-empty reason. It raises the ceiling; it never waives
the requirement for a bound, which is asserted directly.

install-shared.cjs keeps its hand-rolled assert rather than routing through
throwIfFailed: its message embeds both streams, and throwIfFailed carries
only a trimmed stderr. The message now also names the outcome, so a bounded
timeout reads as such across its 38 importers instead of as
expected null to equal 0.

* test(#3145): extract class-norm timeouts and correct the build-hooks sizing

A pre-PR review found 52 copies of four class-norm timeout constants across
this wave. These are not per-suite fixture bindings -- they are shared facts
about how long a class of subprocess takes, derived from a recorded bench
incident. That norm already moved once (60000 to 120000 after a real
ETIMEDOUT), and 52 copies would have drifted the next time it moved.

Extracts tests/helpers/timeouts.cjs, where each norm is justified once, and
converts the copies. A site that genuinely differs -- a real tsc compile, or
regen:derived -- keeps its own local constant with its own justification.

Also corrects a misclassification: scripts/build-hooks.js was sized as a
build at 120000 in twelve places and 60000 in another, but it compiles and
bundles nothing. Its own header says no bundling needed; it copies pre-built
files and syntax-checks them with vm. Three different values bounded one
script; now there is one.

* test(#3145): fix red CI — lint self-match and a Windows chunk overrun

Two failures on PR 3176.

lint-allow-test-rule-refs read a RuleTester fixture as a real exemption. The
fixture exists to prove an unrelated marker does NOT suppress the rule, so it
carries that marker's literal text as test data. Split via concatenation, the
same idiom no-unbounded-spawn-allowlist.test.cjs already uses for its own
self-match problem. The explanatory comment needed the same treatment.

The Windows shard 3/3 chunk was killed at its 600000ms budget. Output stopped
seven minutes before the kill, so this was an overrun rather than a slow
chunk: regenDerivedPropagatesSingleFragmentEditWithNoSecondSourceSurface runs
regen:derived bounded at 900000ms, which is larger than the whole chunk
budget, so the chunk killer always fires first and it can never complete
there. Both the test and that bound predate this change; modifying the file
pulled it into the Windows targeted set and exposed it. Skipped on Windows
with the reason recorded; the Linux lanes cover it. The 900000 bound and its
ceiling marker are unchanged -- they are correct.

* test(#3145): refresh the stale test-timings cost table

The Windows shard was killed at its 600000ms per-chunk budget. run-tests.cjs
packs chunks by measured duration from tests/test-timings.json, and an
unknown file falls back to the table's median weight -- advisory by design,
but it silently underweights exactly the files that matter.

Four of the failing chunk's 22 files were absent from the table, including
the two heaviest: fragment-single-edit-propagation.install.test.cjs at 230s
(it runs regen:derived) and agent-fragments-emission.install.test.cjs at 79s.
Both were weighted as average, so the chunk's total weight read 53.68 against
a budget of 60 and the packer produced a single chunk.

Regenerated from a passing full-suite run, per the remedy the script itself
documents. 700 to 770 entries, 70 added, 0 dropped -- verified, since
gen-test-timings.cjs replaces the table wholesale rather than merging.

Proven against the real packer: the same 22 files now weigh 103.91 and split
into two chunks. No logic, budget, or timeout was changed; raising a budget
to make a red gate pass is not a fix.

27aa40f65e fix(#3023): stage pi's shared hook bundle outside pi's reserved hooks/ directory (#3175)
* test(#3023): failing-first guard — pi must not stage hooks in its reserved dir

pi reserves <configDir>/hooks as its deprecated extension location and warns
on every startup when it exists. Assert a pi install stages the shared hook
bundle under gsd-hooks/ instead, manifests it there, and never creates hooks/.

Also adds pi to the local-scope dir table in install-shared.cjs: pi was in
RUNTIME_META but not LOCAL_DIR_NAME, so scope:'local' resolved
path.join(root, undefined) and no local pi install could be exercised.

Fails before the fix. Verified via the remote runner.

* fix(#3023): stage pi's shared hook bundle outside pi's reserved hooks/ dir

pi reserves <configDir>/hooks as its now-deprecated extension location and
warns on every startup when that directory merely exists — checkDeprecatedExtensionDirs()
guards the warning with a bare existsSync(), unlike its tools/ sibling. GSD staged
its shared hook bundle exactly there, and pi's advised remediation (move it to
extensions/) would break the adapter's paths and expose GSD's .js helpers to pi's
extension auto-discovery.

The bundle directory name is now runtime-descriptor-driven: hostBehaviors
.sharedHooksDirName, defaulting to 'hooks' so all 18 other runtimes are
byte-identical. pi sets 'gsd-hooks'. The name is validated as a single path
segment — separators, dot-only segments, trailing dots, absolute paths, NUL,
and Windows reserved device names all fall back to the default, because the
value is joined onto a user's config root and written to.

Renamed in place rather than relocated: hook scripts resolve siblings via
__dirname/.., so a depth change would silently break them.

- install / uninstall / manifest sites all read the resolved name
- pi/gsd.cjs probes gsd-hooks then hooks, so dev checkouts and half-upgraded
  trees still resolve; the never-throws contract is preserved
- new migration 009 retires the legacy pi hooks/ dir on upgrade, using a new
  non-recursive remove-empty-dir engine primitive (rmdirSync only,
  symlink-refusing, containment-guarded); ADR-0008 amended accordingly
- fixes two latent name-dependencies the rename exposed: the stale-hook scan
  and the injection scanner's self-exclusion both hardcoded 'hooks'

Verified on the remote runner.

* fix(#3023): close review findings and align emitted provenance with the rename

Adversarial review found two defects, and the remote runner found four
failure clusters. All fixed here.

Review BLOCKER — detect-custom-files was blind to the renamed bundle.
GSD_PREFIX_MANAGED_DIRS in gsd-tools.cjs hardcoded 'hooks', so for pi the
whole gsd-hooks/ tree was invisible to the custom-file scan and user-added
files there were never backed up before the next update's clean-install wipe.
The dir set now resolves via the .gsd-runtime marker plus the shipped
capability registry (never bin/install.js, which is not shipped into installed
trees), and falls back to scanning every known candidate when the runtime
cannot be determined — over-scanning is safe, under-scanning is the data loss.

Review MAJOR — the pi adapter bound to an empty bundle. resolveSharedHooksDir
accepted any directory, so an interrupted install left gsd-hooks/ winning over
a fully-staged legacy hooks/ and every hook silently no-opped. A candidate now
qualifies only if it is non-empty.

Remote-runner clusters:
- emitted-provenance had no rule for the gsd-hooks/ family; added two pi-scoped
  rules pointing at the same sources the existing hooks/ rules use. The table is
  total, so an unattributed family is a hard failure by design.
- pi tests in install-minimal-hooks and the install integration suite asserted
  the old layout; updated to derive the dir name from the descriptor rather than
  hardcoding either name.
- 19 unrelated-looking failures on node22 only were a leaked fs mock: t.after()
  runs in registration order, cleanup was registered before mock.restoreAll(),
  and node22's JS rimraf calls the public fs.rmdirSync while node24's native
  path does not — so the EACCES stub leaked process-wide on one lane. Restore
  now runs first.

Verified on the remote runner.

* fix(#3023): honor PI_CODING_AGENT_DIR, ack the rename ripple, fix expandTilde

pi resolves its agent dir as PI_CODING_AGENT_DIR ?? ~/<CONFIG_DIR_NAME>/agent
(packages/coding-agent/src/config.ts). GSD's pi descriptor declared an empty
configHome.env, so a user with that variable set had GSD installed where pi
never looks. Added the env name; the dot-home-nested resolver already handled
the override, so no resolver logic changed.

Also fixes expandTilde in the shared runtime-homes resolver, found while adding
that: it hardcoded os.homedir() and ignored the opts.home every caller threads,
so EVERY runtime's tilde-valued env override (claude, antigravity, windsurf, pi)
silently resolved against the real home. That is a correctness bug and a
test-escape hazard — a sandboxed test asserting on a tilde override reached the
developer's actual home directory. Now threaded through every branch; behavior
with no injected home is unchanged.

Adds the emitted-drift ack fragment for the 58 pi paths whose emitted location
moved with the rename. The provenance rules satisfy the totality gate; the
differential gate needs the ack because the hook sources are byte-unchanged —
only the installer's target directory moved. The two hook files this branch
genuinely edits stay attributed and are not double-acked.

Note on piConfig.configDir: it is read from pi's OWN installed package.json
(getPackageDir walks up from pi's __dirname), alongside piConfig.name — a
white-label setting for a redistributed pi fork, not a per-project user setting.
Documented accordingly rather than treated as an unsupported override.

Verified on the remote runner.

* fix(#3023): reject blank env overrides, pin adapter/descriptor parity

Three review findings, all fixed.

A whitespace-only config-dir override was accepted verbatim: the guard was
`if (val)`, falsy only for the empty string, so PI_CODING_AGENT_DIR='   '
resolved to a literal three-space directory name instead of falling back to the
descriptor default. Fixed across every env-consuming branch — dot-home,
dot-home-nested, all three xdg steps, and generic-agents-root — not just pi's.
Non-blank values are still never trimmed, so '~/My Agent Dir' keeps working.

pi/gsd.cjs's probe list and the descriptor were two independent sources of truth
for the bundle directory name; a future rename would have desynced them silently
and left every pi hook quiet with no error. The probe list stays deliberate — it
must resolve in a dev checkout and a half-upgraded tree, where the registry's
answer would be wrong — so this adds the parity assertion the repo's
generative-fix-divergence rule calls for: the descriptor value must be the FIRST
candidate, and the default must remain present.

Changeset body rewritten to cover the two later user-facing fixes it had not
caught up with.

Verified on the remote runner.

CI's security job flagged CONTEXT.md:124 — pre-existing prose reading 'not the
same fact as a genuinely empty or absent one'. The match was the 'act as a'
INSIDE 'f-act as a': the pattern had no left word boundary, so any word ending
in act tripped it (fact, impact, contract, artifact, interact, redact,
abstract). My four-line CONTEXT.md edit dragged the latent false positive into
this PR because the scan is diff-scoped by file but reads whole files. Anchored
with (^|[^[:alnum:]]) rather than rewording maintainer-owned prose, which would
have left the class alive for the next PR touching any file saying 'fact as a'.

Auditing the rest of the list for the same class surfaced a real detection hole:
the eval/exec/Function patterns matched a quote via \x27, a GNU-grep-only hex
escape. BSD/macOS grep reads it as four literal characters, so single-quoted
eval('...')/exec('...') payloads were NEVER detected there while passing on
GNU-grep CI. Replaced with a literal apostrophe class.

Boundaries were added only where a real word-suffix collision exists; exec,
jailbreak, developer mode and the role-manipulation family were audited and
deliberately left unanchored. 22 new cases cover both directions — the false
positives now scan clean, and every real payload still fires, including the
quote/punctuation/start-of-line boundary forms.

Also builds this branch's injection test fixture at runtime instead of carrying
the literal phrase, so the payload keeps its teeth without tripping the scan.

Verified on the remote runner.

7dd9e59f6b test(#3090): stop exempting violations under categories that do not fit
An allow-test-rule annotation citing a category that does not apply is worse
than no annotation, because it reads as reviewed. Eight were confirmed by
reading the assertions each one covered, and auditing the rest found five more
plus one refutation — a converter test whose wording described the wrong
mechanism while the covered assertion genuinely was deployed-text.

The instructive one used the CANONICAL string for the same mistake: STATE.md
command output labelled as a deployed artifact. A canonical string is not
evidence the category fits, which is why normalising strings alone would have
laundered the problem rather than fixed it. Every mapping the audit had inferred
rather than code-verified was spot-checked before rewriting, and the ones that
turned out not to fit were re-annotated rather than relabelled.

Fourteen STATE.md assertions had a typed extractor available all along and now
use it; their annotations came out because nothing needs exempting. Eight
assertions genuinely need a production change first — CLI stdout and stderr with
no structured mode — and are tagged pending-migration-to-typed-ir citing #3090,
which is what that category is for. It had zero real uses before this, while one
file carried a real citation to migration issue #2974 under a non-canonical tag.

Six annotations covered assertions that do no text matching at all. An exemption
for a violation that does not exist is noise that makes the real ones harder to
audit; those are removed.

atomic-write-coverage gains the annotation it always warranted — its own
docstring describes a structural-regression-guard while the file carried none.

Fifty-nine non-canonical strings across roughly thirty files are normalised, and
the allow-test-rule allowlist is regenerated to match. 472 annotations became
463: every one now uses a canonical category, and the two remaining
non-canonical strings are ESLint RuleTester fixtures, not annotations.

Adds hooks/gsd-write-guard.js, a PreToolUse hook that hard-blocks
(decision: 'block', exit 2) a whole-file Write collapsing a curated
.planning/ artifact (ROADMAP.md, .planning/milestones/*-ROADMAP.md,
STATE.md) below 40% of its on-disk line count. Files under 40 lines
are exempt; GSD_ALLOW_PLANNING_SHRINK=1 (named in the block message)
bypasses for legitimate milestone resets.

Fix 3 of #973 — the only defense independent of per-agent tool config.
Registered on the Claude plugin surface (hooks.json), settings-json
runtimes (runtime-hooks-surface.cts, self-contained pattern), Kimi
spec, and the OpenCode/Kilo plugin buses. Golden install fixtures and
INVENTORY regenerated; regression tests negative-controlled (16/16
RED with the hook absent, 16/16 GREEN with it present).

The guard shipped with Claude-vocabulary checks (tool_name 'Write',
tool_input.file_path), which #2304 showed leaves a guard dormant on
Kimi: the [[hooks]] matcher is registered pre-translated but kimi-cli
forwards its native payload verbatim — tool_name 'WriteFile' (bare or
module-qualified) and tool_input.path per its tool schemas
(src/kimi_cli/tools/file/write.py). The guard matched, saw an unknown
name, and exited 0.

Apply the same per-guard normalization PR #2326 gives the three
sibling guards (name + field mapping, inlined — hook scripts stage as
standalone files), and write the block reason to stderr as well as
stdout JSON: Kimi feeds stderr, not stdout, back to the model on
exit 2, so a stdout-only reason blocks without telling the model why
or naming the documented override.

Regression tests pipe Kimi-shaped payloads (engage, qualified-name,
stderr-reason) plus exemption pins (StrReplaceFile stays out of scope
by design; non-curated paths pass) — verified red against the pre-fix
guard, green after.

Review Blocker 1: the guard hard-blocked /gsd:complete-milestone's ROADMAP

rewrite through a shell write with the hatch set on the command (a hook
inherits the runtime env, so a bare Write cannot carry a per-step override),
and a binding test derives the env var name from the guard's typed output and

catastrophic payload under it — so the next complete-milestone.md edit cannot
silently re-break the wiring.

Review Major 1: StrReplaceFile -> 'Edit' and the old_string/new_string
reconstruction were unreachable-by-effect — the guard exits 0 for any
tool_name !== 'Write', so nothing ever read the fields they set, leaving
guaranteed-surviving mutants against the Stryker bar. The map now carries
only WriteFile -> 'Write'; the StrReplaceFile exemption test message states
the fall-through it actually exercises.

* enhance(#2255): review minors — American spellings; writeSync before exit(2)

Minor 1: normalised/normalise -> American house style. Minor 2: the two
block paths wrote stdout+stderr via async pipe writes then exit(2) —
async-on-Windows, unflushed at exit; fs.writeSync(1/2, ...) makes the block

Minor 3: the last raw-text match in the suite pinned override-name prose on

Review Major 2: the guard's normalizeKimiPayload is a 4th inlined copy with

trivially green until #2326 lands, full-strength after — and the write-guard
copy is bound semantically (map is the value-inverse of convertKimiToolName;

path -> file_path half must be present). Byte-parity is deliberately not
asserted for this copy: it legitimately omits the Edit-class mapping

The committed fixture hashes were generated against a tree predating
next's latest 11 commits, which independently modified the same
install-parity surface. Rebased onto next and regenerated with

Verified: against upstream/next the regenerated fixtures differ by
exactly this PR's own entries -- hooks/gsd-write-guard.js (new),
hooks/managed-hooks-registry.cjs, plugins/gsd-core.js, and
gsd-core/workflows/complete-milestone.md. No unrelated drift.

* fix(#2255): regenerate workflow size baseline for complete-milestone

`complete-milestone.md` grew 31071 -> 32061 (+990) when the round-2
review fix bound GSD_ALLOW_PLANNING_SHRINK=1 into the reorganize step,
but tests/workflow-size-baseline.json was never regenerated. The
per-file workflow baseline test (issue #1074) failed on
ubuntu-latest/22 and both macOS shard 1/3 jobs.

The growth is justified: it is the escape-hatch binding requested in
review round 2 (the guard must not hard-block the tree's only
legitimate milestone reset), not incidental bloat.

Regenerated via `npm run size:baseline`; the diff is exactly the one
entry.

Round-5 M1: the per-step `GSD_ALLOW_PLANNING_SHRINK=1 tee` prefix was inert
(no PreToolUse hook exists on Bash in this family; the write succeeded by
dodging the guard, not by the override firing) and the protection was prose.
The hatch is now a transport code consults: complete-milestone's reorganize
step arms `.planning/.gsd-allow-shrink` with the target's path, keeps the
Write tool as the sanctioned path, and the guard — at the block point only —
verifies the sentinel is fresh (15 min) and names the pending target, then
CONSUMES it and allows that one write. Path-bound + single-use + freshness
keep it from becoming a standing unlock. The env var remains as the
interactive transport, where it can actually reach the hook.

Regression tests written first (negative control: 3 failed pre-fix): the
armed-sentinel Write passes and consumes; stale does not exempt; a token for
a different file neither exempts nor is consumed; the binding test now takes
the sentinel name from the guard's typed output (overrideSentinel), asserts
the step arms it, and asserts the step no longer routes the rewrite around
Write via a shell pipe.

Also in this commit, same file:
- m2: block emission is exception-safe — emitBlock() wraps both writeSync
  sites in their own try/catch that still exits 2, so an EPIPE can no longer
  convert fail-closed into the outer catch's fail-open.
- Header discloses the two reviewed design limits (cumulative sequential
  shrink; lexical match vs symlinked paths) per round-5 scoping.

* docs(#2255): document the sentinel transport across guard surfaces; changeset ends with the (#2255) parenthetical (m4)

USER-GUIDE bullet, INVENTORY row (en + ja/ko/pt/zh), the
runtime-hooks-surface registration comment, and the changeset now describe

Removed rather than wired, per the round-8 review's confirm-and-remove
option. A new binding test asserts the sentinel-armed step is the ONLY
reorganize step in the workflow, so an unguarded collapse step cannot be
silently reintroduced (negative-controlled: fails against the pre-fix
tree). Golden-parity fixtures and the size baseline regenerate for the
shrunk file; every changed fixture key is complete-milestone.md's own.

* test(#2255): document why the read-error injection is a path collision, not an fs monkeypatch

Round-8 nit: the non-ENOENT tests inject via a directory-at-target-path
collision instead of the repo's fs-method monkeypatch pattern. That is
deliberate, not drift — runHook exercises the hook as a spawnSync child
process, so an in-process fs.readFileSync patch (the pattern the cited
siblings use on require'd, in-process code) can never reach the code
under test. Record the reasoning at the injection site.

came back byte-identical, so the merged value was already correct.

attributes to a file this PR changes (complete-milestone.md, the guard,
managed-hooks-registry.cjs, both plugin buses) — derived from the
changed-file set, not a feature-name filter.

* fix(#2255): port the #2595 Kimi-payload hardening the base landed for the sibling guards

Self-found via the round-10 base sweep: a8b40fa5 (#2547/PR #2595) hardened
gsd-prompt/read/worktree-path-guard's normalizeKimiPayload after this PR was
authored, and this guard carried all three pre-fix shapes:

- `path` now wins outright over a model-supplied `file_path` — kimi-cli's
  WriteFile schema carries no `file_path`, so a payload pairing a curated
  `path` with a spurious `file_path: ""` had the guard reading '' and
  exiting 0 while kimi-cli wrote to `path` (one-key bypass, no crash needed).
- null/primitive payloads pass through deliberately instead of throwing into
  the fail-open catch.
- the `file_path` read is typed (`typeof === 'string'`), closing the
  crash-to-allow on `[]`/`{}` — this is what the base-added
  kimi-guard-typed-payload-reads scan flags.

Negative-controlled: the shadowing payload exits 0 against the pre-fix guard
and 2 against this one.

* fix(#2255): round 9 Major 2 — stop printing the sentinel recipe in the agent-facing denial

The block message ended by telling the blocked agent exactly how to arm the
single-use sentinel — handing the #973 actor a mechanical self-authorization
in the moment it is blocked. The recipe is gone from the prose; the
corrective advice and the env override's name stay (the latter is a #2255
acceptance criterion, and a per-step env prefix cannot reach a hook anyway),
and the typed overrideSentinel field stays for the binding tests. The hatch
remains documented in USER-GUIDE.md and complete-milestone.md, where humans
and the workflow engine read.

* fix(#2255): round 9 Minors 1-2 — realpath-resolve the target before the curated match; disclose the /i Linux cost

Minor 1: a Write to a non-curated path that symlinks into a curated file was
not matched while writeFileSync followed the link — the target is now
realpath-resolved before the curated match (ENOENT keeps the lexical
resolution so new-file Writes still pass; any other realpath error falls
through to the read, which fails closed). Negative-controlled: the symlink
payload exits 0 against the pre-fix guard, 2 against this one. Test skips on
win32, where symlink creation needs privilege.

Minor 2: the header's design-limits block now names the unconditional /i
cost on case-sensitive Linux (a genuinely distinct .planning/roadmap.md is
also treated as curated) next to the stateless limit, and drops the closed
symlink limit.

* test(#2255): round 9 Minors 3-4 — CRLF counting pin + a passing Write leaves a fresh sentinel unburned

Minor 3: countLines' split('\n') is CRLF-safe for a count (the \r rides
along), confirmed by trace in the review — this pins it against this repo's
recurring CRLF regressions, on both sides of the compare and at the 40%
boundary.

Minor 4: consumeSentinelFor runs only after the ratio check would block, so
a within-tolerance Write never burns the workflow's token — true by
construction, previously un-asserted.

* fix(#2255): round 9 Major 3 — correct the stale env-var line in archive_milestone's summary

complete-milestone.md's "After archival" bullet still said the reorganize
happens "under GSD_ALLOW_PLANNING_SHRINK=1" — the wording from the round-2

* fix(#2255): round 10 Blocker 1 — arm the hatch in the roadmapper's curated writes

gsd-roadmapper Step 7 Writes both .planning/ROADMAP.md and .planning/STATE.md
wholesale, and /gsd:new-milestone spawns it against the OUTGOING milestone's
files. new-milestone's `phases.clear` archives phase DIRECTORIES, never
ROADMAP.md, so nothing compacts it first, and no ordering rule forces
/gsd:complete-milestone to run before /gsd:new-milestone.

Measured against the shipped hook at the #973 file size (292 lines): a new
4-phase roadmap lands at 18.2% and an 8-phase one at 31.8% — both blocked; only
a 12-phase replacement (45.5%) clears. So the collapse is reachable on a
first-party flow, which is the failure mode #2255 para 4 says the guard must not
have.

Arms the single-use sentinel immediately before each Write, one per target
(the token is path-bound and single-use, so one arming cannot cover both), and
gates each arming on `[ -f ]` so the /gsd:new-project path — where the guard
already exempts via ENOENT — strands no unconsumed token.

Binding test derives the sentinel name from the guard's own typed output, so a
rename on either side fails the suite instead of silently unwiring the hatch.
Negative-controlled: 2 assertions fail against the pre-fix roadmapper.

* docs(#2255): round 10 Major 2 — scope the claim to the guarantee that ships

The sentinel is a plain file, so an agent that would reason past an advisory
can arm one with a single Bash call it is already permitted to make. The
"only defense independent of per-agent tool config" framing outran that, and
the changeset was on its way into CHANGELOG.md.

Retitles the claim on all three surfaces (changeset, guard header, USER-GUIDE)
to what the guard actually delivers: it blocks accidental and single-shot
collapse and is not a defense against a determined agent; what it converts is
"ignore a sentence" into "take one deliberate, path-bound, single-use,
auditable action".

Pinned by test on the DURABLE surfaces only — the guard header and USER-GUIDE.
The changeset fragment is deliberately not pinned: it is consumed at release,
so a test reading it would start failing the moment the release lands. The
bound-statement assertion normalizes comment markers and whitespace first, so
it pins the claim rather than the paragraph's line wrapping.

Negative-controlled: both assertions fail against the pre-fix surfaces.

* test(#2255): acknowledge the roadmapper growth from the round 10 Blocker 1 wiring

The emitted-attribution gate (#2719/#2767) flags gsd-roadmapper.md growing 1130
bytes without an acknowledgment. The growth is the Blocker 1 sentinel wiring
plus the rationale a future editor needs to keep it, so it gets an ack fragment
rather than a silencing regen — the gate's own message is explicit that there is
nothing left to regenerate.

Fragment is PR-scoped (2301-…) per the gate's naming instruction, and uses the
plain-string reason form the shipped fragments use.

Verified against the TRUE upstream tip, not the fork's origin/next: a stale
origin made this same gate report unrelated phantom drift (1 emitted path + 6
grown files + 5 stale acks) that vanishes when GSD_EMITTED_BASE is pinned.

* test(#2255): renumber the roadmapper PROSE_ALLOWLIST pin after the Step 7 wiring

CI red on shard 2/3, all four platforms. The #2751 gate keys PROSE_ALLOWLIST on
{file, line}; the Blocker 1 wiring added 18 lines above the allowlisted
parenthetical in agents/gsd-roadmapper.md, moving it 624 -> 642. Both halves of
the gate then fired: the moved line reads as a new offender, and the stale
entry no longer matches anything.

Line content at 642 is byte-identical to what the entry describes — a
descriptive "e.g." naming SDK queries a user could run — so this is a
renumber, not a re-classification.

Swept the defect class rather than the instance: agents/gsd-roadmapper.md is
the only line-pinned reference to any file this round changed.

Negative-controlled: both assertions fail against the un-renumbered allowlist.

production tree transitively via the Claude Agent SDK and fails the

keeps rollback fidelity (restore re-creates the same link) while never reading
the referent. Scoped the pre-delete to the symlink branch only, so the
regular-file path keeps copyFileSync's overwrite-in-place and a mid-restore
failure cannot destroy the destination. The restore-side existence check moves
to lstat, since existsSync follows a link whose target is gone and would
silently skip the restore.

This lives in the engine all six migrations share, so 000-005 are hardened too.

Also regenerates the pi golden-parity hash: correcting pi/gsd.cjs's own install
docstring changes the extension's content, which the golden suite caught.

missed a fourth: the catch block inside applyInstallerMigrationPlan, which
replays rollback snapshots taken earlier in the SAME apply attempt. Those
snapshots are symlinks precisely because of that commit, so the raw
copyFileSync there dereferenced them and wrote the referent's bytes to the LIVE
install path — worse than the journal-tree leak it was meant to fix, since it
is user-visible and at a predictable location.

Verified by experiment rather than assertion: with the pre-fix line restored,
the managed path comes back as a REGULAR FILE containing the referent's bytes;
with the fix it comes back as a symlink and the bytes appear nowhere.

The accompanying test injects the failure by letting the delete succeed and
then throwing once, modelling a later step failing after the delete. That
ordering is load-bearing — an earlier draft injected before the delete, which
leaves the live path in place, so the pre-fix copyFileSync hit a same-file
collision and threw instead of leaking. That draft passed against the bug it
was written to catch; this one fails against it.

Adds the missing rollback() coverage as well: a restored symlinked managed path
must come back as a link pointing at its original target.

AND hostBehaviors.skipSharedHooksInstall:true, which suppresses staging
of hooks/*.js into the Kilo config dir. The plugin's runHook treats an
absent hook script as a silent allow, so every guard it spawned
(gsd-prompt-guard, gsd-read-guard, gsd-worktree-path-guard) no-opped on
every Kilo install. OpenCode uses the byte-identical plugin with hook
staging on and is unaffected — it is the reference shape.

The skip flag predates Kilo's plugin surface: it dates to #1821 (hooks
were dead weight for a runtime with no hook consumer), and #2093 added
the hooks-dependent nativePlugin without revisiting it.

- capabilities/kilo/capability.json: remove skipSharedHooksInstall
  (regenerated gsd-core/bin/lib/capability-registry.cjs accordingly)

79d7657eff feat(#2102): make pi a first-class installable runtime + fix its dispatch (ADR-1239)
Net-new EoS/pi installable runtime — purely additive (no prior runtime==='pi'
branches). pi is a bun-runtime programmatic-CLI whose /gsd command is registered
by a native ExtensionAPI extension and dispatches through the embedded engine.

Stage 1 (install plumbing):
- capabilities/pi/capability.json: full hostIntegration descriptor (imperative /
  slash-programmatic / active-model / native-extension / bun) + hostBehaviors
  {nativePlugin, pluginOnlyInstall}.
- --pi flag + interactive-menu renumber (All 17->18); pi added to RUNTIME_FLAG_IDS,
  RUNTIME_LABELS, RUNTIME_META, allRuntimes/runtimeMap, model-catalog defaults.
- Install mirrors OpenCode: pi installs the gsd.cjs extension + the shared engine
  payload (gsd-core + scripts + config markers) + the shared hooks bundle (spawned
  by the extension at lifecycle events, like OpenCode's plugin). pluginOnlyInstall
  EXCLUDES declarative command/agent/skill markdown, which pi has no host-read
  surface for (its /gsd is programmatic). _installNativePluginIfDeclared (extracted
  from the opencode-family path) copies pi/gsd.cjs -> ~/.pi/agent/extensions/gsd.cjs
  (global) / .pi/extensions/ (local). pi added to package.json files.
- Golden: new pi.json (320 files: extension + engine + 27-file hooks bundle, no
  markdown); the 16 other fixtures + claude-local change only by the shared
  model-catalog hash line.

Stage 2 (real dispatch + upgrades):
- Shared dispatchGsdCommand() (shell-command-projection): bounded, no-throw
  subprocess-shim to gsd-tools.cjs (the only full-surface dispatch path; no
  in-process full-hub factory exists). Fixes pi/gsd.cjs's createHub()-no-args bug
  (every dispatch was UnknownCommand) AND the identical bug in mcp-server.cts's
  gsd_invoke_command, which a vacuous unknown-family-only test had masked (now has
  a real dispatch regression test).
- pi/gsd.cjs: /gsd handler now (args, ctx) - tokenizes (quote-aware, via the
  shipped hooks/lib/git-cmd.js) + dispatches real family/subcommand (not hardcoded
  query/help); gsd_invoke gets a TypeBox (JSON-schema-fallback) parameters schema +
  consumes params; getArgumentCompletions; before_provider_request active-model
  steering (fail-open on null resolution); functional session_start /
  before_agent_start / session_before_compact hook bridges (spawn the shipped GSD
  hook scripts).
- EXTENSION_EVENT_SURFACES.pi expanded from ['tool_call'] to the full 30-event
  vocabulary.

6e773d97df feat(#2088): migrate Codex onto the Embeddable Orchestration System (ADR-1239)
Drive Codex install/uninstall through the descriptor-driven Host-Integration
Interface (declarative embedding adapter → engine surface dispatch) and fold
every positive `runtime === 'codex'` / `isCodex` projection into descriptor-driven
`runtime.hostBehaviors`. Install/uninstall output stays byte-parity-gated
(tests/fixtures/golden-install-parity/codex.json); no other runtime changes.

Three Context7-verified upgrades, each with a test on the user-reachable surface:
- Skill root → canonical $HOME/.agents/skills via a skills-kind `home` override,
  with pre-move migration cleanup (stale ~/.codex/skills/gsd-* removed on install
  and uninstall; user content preserved). Fixes getGlobalSkillsBase, writeManifest,
  and the skill-manifest inventory to honor the override so --skills-root /
  sync-skills / the manifest report the real location.
- Six new hooks.json lifecycle events (PreToolUse, PermissionRequest, PreCompact,
  PostCompact, SubagentStop, UserPromptSubmit) shared by install + uninstall;
  extendedHookEvents reconciled [] -> the schema-valid wired subset.
- Explicit `[agents] max_depth = 1` in the managed config.toml block, pinning the
  negotiated dispatch.maxDepth:1 axis. validateCodexConfigSchema now permits a
  known-scalar-only bare `[agents]` AgentsToml table (still rejects [[agents]] and
  unknown-key break-forms, #2760); mergeCodexConfig preserves the user's own
  AgentsToml scalars (max_threads etc.) instead of dropping them.

2b806904e2 fix(#1821): stop copying dead hook scripts for Kilo and ZCode (hooksSurface:none)
Kilo and ZCode both declare `hooksSurface: 'none'` and have no plugin surface,
so the GSD installer staged lifecycle hook scripts (hooks/*.js, hooks/*.sh,
hooks/lib/) plus a `{"type":"commonjs"}` package.json marker into their config
dirs where nothing ever invokes them — dead weight (#1821).

The installer's two hook-copy guards at bin/install.js were still on the legacy
hardcoded runtime-name list and never excluded Kilo or ZCode. Add
`&& !isKilo && !isZcode` to both (isZcode added to the install() runtimeFlags
destructure).

OpenCode — which #1821 also named — is deliberately NOT excluded: since the
issue was filed, #1914 shipped a native OpenCode plugin (plugins/gsd-core.js)
that spawns those exact staged hooks via OpenCode's event bus and requires both
the hook scripts and the CommonJS package.json marker. Excluding OpenCode would
regress #1914, so its hooks stay live. The genuinely-dead cases are Kilo & ZCode.

- bin/install.js: add `&& !isKilo && !isZcode` to the hooks/dist copy guard and
  the hooks/lib copy guard; document the OpenCode-vs-Kilo/ZCode split.
- tests/install-minimal-hooks.test.cjs: regression test asserting Kilo and ZCode
  receive no gsd-*.js/.sh hooks or hooks/lib, while OpenCode keeps its hooks +
  #1914 plugin and Claude keeps its hooks (over-exclusion guard).
- tests/fixtures/golden-install-parity/{kilo,zcode}.json: drop the 21 hooks/*
  entries and the package.json marker they no longer receive (opencode unchanged).

6d072435d0 test(#1975): consolidate 51 CLI + scripts-tooling regression tests into module suites
Fold 51 issue-named CLI black-box + scripts-tooling regression files into their
canonical module suites (runtime-launcher-parity, worktree-safety, install-*, managed-hooks,
read-guard, capability-registry, etc.), plus a NEW slash-command-namespace.test.cjs grouping
the 4 slash/colon-namespace-leak invariant suites that had no canonical owner. Verbatim
block-scoped describe wrappers; 427 subtests conserved 1:1.

Host-env pre-check (per B2): no CLI-receiving host sets a redirecting GSD_WORKSTREAM/GSD_PROJECT
value. One folded suite (bug-3668 runtime resolver) creates an extension-less PATH gsd-tools
stub + bash -c; co-locating it with the host's chmodSync tripped local/no-unguarded-nonportable-exec,
so it's now Windows-guarded (skip on win32) matching the host suite's own bash -c guard.

Regenerates regression-name allowlist (222->182), ratchets file-count allowlist (graphify 7->6,
docs entry removed), makes 26 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456;
prunes stale ids). Repoints 13 tests/ references across CONTEXT.md, COMMANDS.md/FEATURES.md
(EN + ja/ko/pt/zh) and ADR-0002. lint:ci green.

../ so it resolves from tests/ root (verified). Host-env pre-check: no CLI-receiving host
sets a redirecting GSD_WORKSTREAM/GSD_PROJECT value.

Fold 61 issue-named install/codex/runtime regression files into the canonical
test file that owns each subject-under-test, preserving every assertion and its
origin issue number as provenance (block-scoped describe wrappers, zero assertion
loss — 652 subtests conserved 1:1). Routes:

- codex-config.test.cjs        +19 (codex config/toml/hooks/adapter/skill surface)
- install.test.cjs             +18 (node-runner norm, manifest, arg parse, finishInstall)
- install-runtime-artifacts    +13 (per-runtime conversion + emission)
- install-minimal-hooks        +6  (hook-event dialects + guards)
- path-replacement             +2  (opencode absolute pathPrefix)
- install-write-confinement    +2  (pristine dir writes)
- install-regressions          +1  (user-artifact preservation)

Removes 61 tests/ files → 61 fewer CI processes. Regenerates the regression-name
allowlist (271→222), ratchets the file-count allowlist (config 10→9, install 12→9),
and makes 13 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456;
prunes 13 stale allowlist ids). Repoints ADR-0009's moved-test list. lint:ci green.

multi-family inventory table) are left intact, each with a one-line comment.

Test-only; no production code touched.

0a11d361ca feat(#69): nest concrete skills under namespace routers at install (#883)
Emit the 6 gsd-ns-* routers as the only top-level skill bundles and nest
the ~61 concrete skills under <router>/skills/<name>/SKILL.md on runtimes
with confirmed non-recursive skill loaders (claude global, cline, qwen,
hermes, augment, trae, antigravity). Router bodies rewrite their routing
tables from Skill-tool dispatch to a Read skills/<name>/SKILL.md pattern.
Recursive/unconfirmed loaders (cursor, codex, copilot, windsurf, codebuddy,
opencode, kilo) keep the flat layout. Completes the v1.40 namespace
architecture (#2792) so the eager skill listing drops to ~6 entries.

The installer --help text undercounted profile skill counts: it claimed
"core — 7 main-loop skills" and "standard — ~13 skills" when the real
counts in PROFILES (bin/lib/install-profiles.cjs) are 8 and 14. The
`surface` skill was added to core after the help string was written, and
standard was never updated.

Derive the core and standard counts from PROFILES.core.length and
PROFILES.standard.length so the help text cannot drift again. Replace the
stale hardcoded "all 66 skills" (actually 67) on the full line with
drift-proof "all skills" — full is the '*' sentinel with no array length
and no cheap authoritative total in the help path.

Add regression tests that run `node bin/install.js --help` and assert the
printed core/standard counts equal the PROFILES lengths, plus a guard
that the full line carries no hardcoded numeric count.

No behavior change. Lint 345→125 warnings (0 errors); deferred categories (n/no-process-exit, test-sleeps, control-regex) tracked in #732 for follow-up. Full test suite green (0 failures); code-review verified all removals unused and all escape fixes semantics-preserving.

Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the
on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary
(`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers
are unaffected.

Mechanical (bulk, ~90% of the diff):
- `git mv get-shit-done gsd-core`
- Swept path/identifier references across the repo via
  `perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead
  preserves the five legitimate slug variants that are NOT the directory:
  get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names).
- Build/manifest wiring: package.json (bin, files, coverage globs),
  tsconfig.build.json (outDir), ~86 .gitignore build-output entries,
  stryker.config.mjs, scan-ignore files, install.js path strings.
- Frozen (not rewritten): CHANGELOG.md history; translated docs
  (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/).

New logic (review here):
- src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper
  ADR-0008 installer migration. On upgrade it walks the legacy
  `~/.claude/get-shit-done/` tree, classifies each file via the prior install
  manifest, and emits remove-managed / backup-and-remove for managed files
  while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked
  root and symlinked entries; bounds-checks every path under configDir). The
  framework rolls back on install failure. Emptied dirs may remain (framework
  has no recursive dir-removal primitive) — documented.
- scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare
  `get-shit-done` directory token (split token to avoid self-match; case-
  insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists
  CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines).
  Wired into the lint-tests CI job.
- Restored scripts/lint-package-identity-drift.cjs detection regexes (the
  mechanical sweep had wrongly rewritten the old-name patterns it exists to
  detect) and marked them as intentional legacy references.
- TDD tests for the migration and the guard; do.md slash-command guard regex
  tightened so a `/gsd-core/bin` path segment is not mistaken for a command;
  changeset + docs/installer-migrations.md row added.

Breaking: the installed runtime path moves `~/.claude/get-shit-done/` ->
`~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed
files (preserving user files) on upgrade. Users with custom hooks/configs
hardcoding the old path must update them.

a28dcec981 chore(#597): replace count-based ratchet guards with AST lint + named-set allowlists (#603)
The windows-test-parity ratchet greps test source for fs.rmSync-without-
maxRetries (and six other Windows-portability anti-patterns), failing when an
integer offender COUNT exceeds a frozen baseline (rmSync: 95). A count ratchet
is a Goodhart metric: fixing one offender and adding another keeps the count
constant, so a new defect slips through green. Replace it — and every other
count ratchet in the repo — with a layered, masking-proof design.

Behavioral seam test
- tests/helpers-cleanup.test.cjs proves helpers.cleanup() carries the Windows
  EBUSY retry budget. cleanup() delegates retries to Node's fs.rmSync via
  maxRetries (it owns no loop), so the test asserts the option contract
  (recursive/force/maxRetries>0/retryDelay>0) + real-FS removal + the cwd-guard,
  rather than a loop that does not exist. The EBUSY risk is now tested ONCE at
  the helper, not approximated textually at every call site.

Write-time ESLint rule (AST-accurate, replaces the grep)
- eslint-rules/no-raw-rmsync-in-tests.cjs (error in tests/**/*.test.cjs) bans
  raw fs.rmSync, steering to cleanup(). Catches member, computed (fs['rmSync']),
  destructured and aliased forms; escape hatch is inline
  `// eslint-disable-next-line local/no-raw-rmsync-in-tests -- <reason>` only.
- Migrated 336 raw fs.rmSync teardown calls across ~116 test files to cleanup().
  ~18 genuinely load-bearing sites (mid-test SUT/fault-injection removals,
  error-swallowing or name-colliding local teardown helpers) keep the raw call
  with an inline eslint-disable + reason.

Shared anti-ratchet primitive
- scripts/lib/allowlist-ratchet.cjs:
  - assertWithinAllowlist: fails on NOVEL ids (new offender introduced) AND on
    STALE ids (a known offender was fixed but not pruned) — identity, not count,
    and a ratchet DOWN toward zero.
  - assertTightCeiling: a size/length budget whose ceiling must stay within a
    grace band of the high-water mark, so budgets may only tighten, never creep.

Ratchets converted onto the primitive
- windows-test-parity-guard.test.cjs: rmSync rule deleted (now ESLint-enforced);
  the remaining six patterns moved from integer baselines to named-set
  allowlists with ratchet-down.
- scripts/lint-test-file-count.{cjs,allowlist.json}: per-module integer counts →
  named filename sets (closes the swap-a-file-keep-the-count blind spot); a
  module dropping under cap now FAILS to force pruning its allowlist entry.
- enh-2790 skill-count `<= 63` → named skill allowlist (ratchets toward ~58).

Size budgets hardened (tighten-only)
- agent-size / workflow-size / feat-3039 help-tiered: ceilings lowered to the
  current high-water mark and an assertTightCeiling anti-creep check added per
  tier. Fixed external-contract limits (description ≤100 chars, agent ≤100 KB)
  are intentionally left as-is — they are not grandfathered creeping budgets.

No user-facing behavior change (tests + tooling only); no USER_FACING_PREFIXES
touched, so no changeset fragment is required.

f955bf872c refactor(tests): consolidate Installer Module — 11 files → 2 (#3760)
* chore(tests): lint rule — cap test files per production module at 2

Adds scripts/lint-test-file-count.cjs with a ratcheted allowlist
(scripts/lint-test-file-count.allowlist.json) capturing today's
30 violating clusters as a ceiling. New entries blocked at PR time;
reductions ratchet automatically.

Wires into .github/workflows/test.yml as a new step in lint-tests.

* refactor(tests): split install.test.cjs along section seams — keep files ≤ 800 LOC

1828-LOC monolith split into 3 files by semantic boundary:
  install.test.cjs (602 LOC) — S1-5: dir resolution, install/uninstall spot-checks, Kilo
  install-runtime-artifacts.test.cjs (347 LOC) — S6-8+12: layout loop, Contract 6, legacy migrations
  install-minimal-hooks.test.cjs (782 LOC) — S9-11+13: profiles, minimal E2E, hooks copy

Shared constants and helpers extracted to tests/helpers/install-shared.cjs (216 LOC).
Total test count unchanged: 210 tests (68+37+105).

* fix(tests): ratchet state test-file ceiling to 9 — actual count at baseline

The allowlist was initialised with state=8 but the repo already had 9 files
matching the 'state' prefix at the time the lint rule was created, causing
lint-test-file-count.test.cjs to fail on the real repo immediately.
