91f5bd7cb1 feat(#607): rebuild get-shit-done-cc → gsd-core migration (per-package cache + installer auto-cleanup + --dry-run) (#611)
* feat(#607): rebuild get-shit-done-cc → gsd-core migration

Leftover get-shit-done-cc installs poisoned the shared update cache,
causing a permanent false "update available". Rebuild the migration so a
stale old install is both harmless and actively removed.

- Per-package update cache filename (gsd-update-check-<slug>.json) in the
  shared ~/.cache/gsd dir, single-sourced via package-identity; writers
  stamp package_name and readers reject foreign/absent lineage. Multi-
  runtime visibility preserved (same shared dir + filename across runtimes).
- New get-shit-done/bin/lib/legacy-cleanup.cjs seam: detects code-file
  references to the old package + the legacy fixed-name cache across home
  runtime dirs; installer auto-cleans on every install; --dry-run previews
  and mutates nothing. User hooks and dev-preferences are never touched.
- update.md cache-clear globs gsd-update-check*.json across ALL supported
  runtimes (adds cursor/windsurf/augment/trae/qwen/hermes/codebuddy/cline).
- Fix worker MODULE_NOT_FOUND post-install (ship managed-hooks-registry.cjs
  + degrade gracefully) so the per-package cache is always written.
- Diataxis how-to: docs/cleanup-get-shit-done-cc.md.

* fix(#607): set USERPROFILE alongside HOME in dry-run install test for Windows

os.homedir() reads USERPROFILE on win32, so HOME-only isolation let the

0c1084ba88 fix(#48): verify-only worktree_branch_check + orchestrator cwd-drift guard (#590)
Closes #48. Makes the canonical worktree_branch_check fragment verify-only/fail-closed (exit 42, no git reset self-recovery), adds an orchestrator fail-closed collection rule and a cwd-drift guard at execute_waves entry. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

b88d6d6ef1 refactor(#588): consolidate duplicated worktree_branch_check into one canonical fragment (#589)
Extracts the fail-closed worktree branch-check guard into a single canonical fragment (get-shit-done/references/worktree-branch-check.md) and repoints all five sites at it; orchestrator embeds the runnable block at dispatch. All safety invariants preserved; adversarially reviewed; full matrix green. Closes #588.

f069331737 Enhancement(#41): /gsd-ship extracts per-commit gate_status into a PR-body TDD Audit + squash trailer (#585)
* feat(#41): extract per-commit gate_status into ship PR body TDD Audit

/gsd:ship's generate_pr_body now reconstructs the TDD gate trail that a
squash-merge would otherwise discard. A new TDD Audit section walks the
merge-base..HEAD commit range (merges excluded), reads each commit's
gate_status: trailer via Git's native trailer machinery, pairs each
test: commit with its following feat:/fix: implementation commit, and
counts commits lacking a recognized trailer as missing. A single
aggregate `gate_status: skill=N, fallback=N, exempt=N, missing=N`
trailer is emitted as the final line of the PR body so a GitHub
squash-merge carries the audit footprint into the base branch.

Hardening (per adversarial review): impl pairing is restricted to
feat:/fix: (refactor/docs/chore are skipped, never mistaken for GREEN);
the gate_status cell is normalized to a known token and never rendered
raw; commits with multiple gate_status trailers are treated as missing;
every table cell escapes pipes and strips CR/LF; records guard against
delimiter-injection from adversarial commit messages.

Scoped additively: no changes to commands, agents, templates, or SDK.

Closes #41

c2ebb1ba16 fix(#571): forbid Write in doc-writer fix mode; add workflow truncation guard
* fix(#571): forbid Write in doc-writer fix mode; add workflow truncation guard

gsd-doc-writer in fix mode only had Write in its tools list, so when
correcting a specific failing claim it would re-emit the whole file with
only the lines it had in context — truncating untracked docs with no git
recovery path.

Fix 1 (root cause): add Edit to the agent tools frontmatter and rewrite
fix_mode instructions to mandate Edit for surgical corrections and
explicitly forbid Write on existing files. Also reinforced in
critical_rules.

Fix 2 (safety net): add a post-fix line-count guard in the fix_loop step
of docs-update.md. If the file shrank by >90% after a fix agent runs,
the orchestrator restores the file from the existing_content it captured
before dispatch and logs a WARNING. This makes the previously
unrecoverable case recoverable.

Regression test: tests/bug-571-doc-writer-fix-mode-edit-only.test.cjs
covers both the agent contract and the workflow guard.

e3bc53f835 enhancement(#558): add liveness hints to all GSD spawn announcements (#566)
* enhancement(#558): add liveness hints to all GSD spawn announcements

Append '(runs in a subagent — no output until it returns, ~1–5 min; expected,
not a freeze)' inline to every ◆ Spawning… banner and subagent dispatch
instruction across 26 workflows. Silent subagents look identical to frozen
sessions — this note sets the expectation so users wait instead of killing
healthy in-progress work.

Changes:
- references/ui-brand.md: document liveness convention under Spawning Indicators
- 10 banner workflows: append liveness note to ◆ Spawning… lines in-place
- 18 subagent-only workflows: add print instruction with liveness phrase
- tests/spawn-liveness-banner.test.cjs: new test; fails if any workflow with
  subagent_type omits 'runs in a subagent'
- docs/USER-GUIDE.md: troubleshooting entry for frozen-looking spawns
- .changeset/558-spawn-liveness-banner.md: changeset fragment

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#558): add missing pr field to changeset fragment

The changeset lint requires pr: <NNN> in frontmatter; the fragment was
written without it, causing parse.cjs to reject it.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#558): address codex review — missed spawns and tighten test

- plan-phase.md: add liveness note to chunked outline planner and
  per-plan chunked planner banners (two missed ◆ Spawning… lines)
- quick.md: add liveness note to research banner and add missing
  display line before planner spawn in Step 5
- plan-review-convergence.md: add liveness note to initial planning
  and review-agent spawn Display lines
- docs-update.md: add Print instructions with liveness note before
  gsd-doc-verifier spawns in Phase 1 and Phase 2
- autonomous.md: add Print instruction with liveness note before
  background plan-phase agent dispatch in step 3b
- tests/spawn-liveness-banner.test.cjs: replace single file-level
  check with two assertions:
  (1) every ◆ Spawning… banner line carries the phrase on that line
  (2) every file with subagent_type contains the phrase somewhere
  The tighter test would have caught all five missed spawns.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#558): tighten spawn-liveness test regex to catch spawn-word-anywhere variants

Previous SPAWN_BANNER_RE only matched ◆ immediately followed by Spawning|spawning.
Replace with /◆[^\n]*\bspawning?\b/i which matches the spawn word anywhere on the
◆ line — catching "◆ Chunked mode: spawning outline planner..." and similar.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#558): rename changeset to PR number 566 and correct pr field

Changeset was filed as 558-spawn-liveness-banner.md (issue#) but the
convention is the PR number. Renamed to 566-spawn-liveness-banner.md
and updated pr: 558 → pr: 566 so release notes link to the right PR.

48c1827028 enhancement(#40): integrate branch pruning into /gsd-cleanup archival workflow (#564)
* enhancement(#40): integrate branch pruning into /gsd-cleanup archival workflow

Adds a prune_local_branches step to cleanup.md (between archive_phases and
commit) that force-deletes local branches whose upstream is gone — keeping
local clones symmetric with delete_branch_on_merge on GitHub.

Key design choices vs. PR #562 (the local-model draft):
- dry-run step shows stale branches using cached tracking refs only; git
  fetch --prune is deferred to the execution step so the dry-run is
  non-side-effecting
- awk uses { if ($1 != "*") print $1 } form to explicitly exclude the
  currently checked-out branch (the * prefix in git branch -vv output),
  not a prose note that lets xargs receive literal * as an argument
- git fetch --prune runs exactly once, in prune_local_branches, eliminating
  the TOCTOU window between a preview fetch and an execution fetch
- two new negative-contract tests: identify_completed_milestones must not
  run git branch commands; show_dry_run must not run git fetch --prune

Closes #40

* chore: regenerate changeset using repo script (correct format)

Replaces hand-written fragment (used `/** ... */` comment syntax)
with one generated by `npm run changeset -- --type Changed --pr 562`.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix: address codex review blockers — protect main/next/trunk, align dry-run with execution

Codex adversarial review (pre-PR gate) flagged two blockers:

1. awk filter only excluded '*' (current branch) but not protected names.
   main/next/trunk/develop could be force-deleted if their upstream was
   gone. Fix: use !~ /^\*$|^main$|^next$|^trunk$|^develop$/ regex match.

2. Dry-run enumerated from cached tracking refs; execution re-ran
   git fetch --prune, creating a TOCTOU window between what the user
   confirmed and what got deleted. Fix: move git fetch --prune into
   show_dry_run (prefetch for display accuracy); prune_local_branches
   now enumerates from the already-fetched state with no second fetch.

Updated 14 structural tests to match new design (added protected-name
exclusion test; inverted show_dry_run fetch assertion).

c976a9c858 fix(#38): replace misleading approved checkpoint in execute-phase human_needed branch (#560)
Fixes #38

Removes the `"approved" → continue` ack-and-advance shortcut from the `human_needed` verification path in execute-phase. The phase now stays pending until `/gsd:verify-work` completes the UAT and triggers its auto-transition — enforcing the invariant that ROADMAP advances only after a completed verification record.

Also fixes: UAT file format mismatch (`status: testing` + correct `## Current Test` key shape), filename alignment (`{phase_num}-UAT.md`), explicit ack handler covering legacy `approved` keyword, stale `HUMAN-UAT.md` references in agent/reference files.

6fc84528e5 fix(#448): resolve UI safety gate helper against the GSD install dir (#539)
* fix(#448): resolve UI safety gate helper against the GSD install dir

The §5.6 UI Design Contract Gate (and autonomous §3a.5) resolved
ui-safety-gate.cjs via `git rev-parse --show-toplevel`, i.e. the
consuming project's git root — which has no bin/lib. The node call
failed, its exit code was conflated with "no UI", and the gate
silently no-opped so frontend phases skipped the UI-SPEC prompt.

Resolve the helper against the GSD install dir via RUNTIME_DIR (the
same idiom §1 uses for gsd-tools), with git-toplevel and $HOME/.claude
fallbacks. When the helper genuinely can't be found, fail OPEN with a
stderr warning (assume UI present) rather than silently skipping.

Tests: bug-3706 structural guard now requires RUNTIME_DIR resolution
and forbids the consuming-project GSD_REPO_ROOT anchor; a new
behavioral test resolves and runs the helper from a temp consuming
project (no bin/lib) with RUNTIME_DIR set. autonomous-ui-steps updated
to match.

The installer copies get-shit-done/ to the target but not root bin/lib/, so
the helper was never found for installed users. Placing ui-safety-gate.cjs in
get-shit-done/bin/lib/ ensures the installer deploys it, and probing that path
first makes the gate work correctly in installed runtimes.

3d39064406 fix(#447): scope post-planning gap analysis to phase_req_ids (#538)
* fix(#447): scope post-planning gap analysis to phase_req_ids

§13e gap-analysis diffed the entire REQUIREMENTS.md against a phase's
plans even when the phase mapped no REQ-IDs, so a phase mapping nothing
reported every unrelated project requirement as "not covered".

Teach the gap-analysis CLI a --phase-req-ids option (null/TBD skips the
requirements comparison, an ID list scopes to it, absent = unchanged
back-compat) and have §13e pass the phase's mapped IDs — mirroring the
§13 Requirements Coverage Gate's null/TBD skip. CONTEXT.md decisions stay
in scope regardless.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#447): source phase_req_ids from init.plan-phase, not roadmap.get-phase

Adversarial-review follow-up. roadmap.get-phase returns the raw phase
markdown (not JSON), so `--pick phase_req_ids` yielded nothing and §13e
would have skipped the requirements comparison for EVERY phase — a
silent regression. phase_req_ids is exposed by init.plan-phase; switch
§13e to it. Adds an integration test asserting the query exposes the
IDs and that gap-analysis scopes to them (and skips when unmapped).

05cdec5f47 feat(#22): plan-vs-codebase drift guard (source-grounded reviewer + intel surface) (#487)
* feat(#22): add plan_review.source_grounding + _authority config keys

Two additive opt-out keys for the drift guard: source_grounding (bool,

grep|intel|treesitter|lsp|scip, default grep) selects the resolver rung.
No existing default changed.

* feat(#22): add intel api-surface renderer + CLI subcommand

Renders .planning/intel/api-map.json into a human-readable API-SURFACE.md
for planner injection. Empty/missing map still writes a surface that
announces itself incomplete (absence = unknown, not 'does not exist').
Gated on intel.enabled like all intel functions.

Refs #22

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* feat(#22): add source-grounding pass to plan-review-convergence

Default-on reviewer pass (plan_review.source_grounding) that enumerates
every symbol a plan cites, excludes declared new artifacts, resolves each
against source via the configured authority adapter, and records
three-valued verdicts. rung-0/1 MISSING is needs-acknowledgement, not a
hard block; UNCHECKABLE is logged in a REVIEWS.md coverage section.

Refs #22

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* feat(#22): inject API-SURFACE.md into planner + require Artifacts section

When intel.enabled, plan-phase regenerates API-SURFACE.md and injects it
as a HINT (prefer, may be incomplete, absence = unknown), never a hard
rule. Every plan must now emit an 'Artifacts this phase produces' section
so the source-grounding reviewer can separate new symbols from references
to existing code.

* feat(#22): surface drift-guard in setup + settings, add docs

/gsd:new-project asks to enable plan_review.source_grounding (default Y);
/gsd:settings exposes the toggle and authority knob. Documents both config
keys in CONFIGURATION.md, the intel api-surface command in COMMANDS.md,
the drift guard in USER-GUIDE.md, and links ADR 22 from ARCHITECTURE.md.

* fix(#22): respect AskUserQuestion 4-option cap and plan-phase XL line budget

settings drift-guard toggle moved to its own 2-option question; #22
plan-phase additions condensed to bring the file back under the 1810-line
XL budget without dropping the intel gate, the incomplete-surface hint, or
the Artifacts-section requirement.

* fix(#22): use live slash-command forms in drift-guard docs

Doc-parity gate requires every slash-command token in docs/*.md to resolve
to a registered command. Corrected the command form(s) referenced in the
#22 drift-guard / api-surface documentation.

The unresolved token was /gsd-core, matched from the GitHub repo reference
"open-gsd/gsd-core#22" in docs/adr/22-plan-drift-guard.md. This is the
same pattern as the existing 'test-runner' exemption (open-gsd/gsd-test-runner).
Added 'core' to INTERNAL_COMPONENT_SLUGS with a matching explanatory comment.

6f2520786d feat(#498): single Package Identity seam for /gsd:update + fix runtime undefined-name bug (#499)
* feat(#498): generated package-identity seam derived from package.json

Introduce a single source for GSD's published-package coordinates:
scripts/generate-package-identity.cjs (pure deriveIdentity + formatManualInstall
+ render) emits the generated get-shit-done/bin/lib/package-identity.cjs with
values baked from package.json at build time. Baking is required because the
installed tree carries only a synthetic {"type":"commonjs"} package.json, so a
runtime require('package.json').name resolves to undefined (#378). Reconciles

Wired into npm run build; a parity test fails CI if the committed file drifts
from package.json.

Refs #498

* fix(#498): repoint update worker + check-latest-version at the seam

- check-latest-version.cjs sources PACKAGE_NAME from the package-identity seam
  instead of a re-typed literal (single source; #2992's constant guarantee is
  preserved since the seam bakes from package.json).
- gsd-check-update-worker.js no longer does require('../package.json').name
  (resolved to undefined in the installed tree → background update check
  silently broken, #378). It now delegates the latest-version lookup to
  checkLatestVersion(), collapsing the duplicated npm-view call onto the single
  deterministic adapter and inheriting its typed {ok,version,reason} surface.
- Move the PR #3102 Windows shell-gate contract test onto execNpm (where the
  spawn now lives) and assert the worker no longer spawns npm directly.
- Rewrite the #378 contract: worker must NOT use require(package.json).name and
  must delegate; check-latest-version PACKAGE_NAME is single-sourced from the seam.

* feat(#498): drift-guard lint — value-check GSD coordinate literals against the seam

scripts/lint-package-identity-drift.cjs scans the runtime/code surface
(bin/, hooks/, scripts/, get-shit-done/) and asserts every GSD package name
and GitHub repo slug literal equals the Package Identity seam's current value.
Passes today; fails the moment a repoint isn't propagated (rename package.json,
regenerate the seam, and stale literals are reported until updated). This is
the second adapter that makes the seam real and a repoint mechanically safe.

Enforced via tests/issue-498-identity-drift-lint.test.cjs (scanRepo === [])
under npm test; also exposed as `npm run check:identity-drift`.

* feat(#498): update-context projection — port update.md resolution to a tested seam

Add get-shit-done/bin/lib/update-context.cjs: a pure, injected-fs port of
update.md's ~280-line get_installed_version bash. resolveUpdateContext()
reproduces the full precedence cascade (preferred fast-path -> local probe ->
global probe via env overrides then $HOME -> LOCAL-if-distinct -> scope
cascade -> UNKNOWN) and returns the 4-field contract { installedVersion,
scope, runtime, gsdDir }. The fs is injected so every branch is finally
testable without a live multi-runtime install.

Expose it as `gsd-tools update-context [--config-dir <d>] [--runtime <r>] --json`.
Purely additive — update.md is unchanged in this commit; the workflow swap
follows separately.

* feat(#498): swap update.md resolution to the update-context projection

Replace ~280 lines of inline runtime/scope/config-dir bash in update.md's
get_installed_version step with a call to `gsd-tools update-context --json`
(60 lines: derive PREFERRED_* from execution_context, resolve gsd-tools.cjs,
parse the 4-field JSON). Behavior is unchanged — the projection reproduces the
same cascade — but the logic is now tested in update-context.cjs instead of
untestable bash-in-markdown.

Relocate the #3608 antigravity-first-class contract onto the projection
(RUNTIME_DIRS order, inferPreferredRuntime, envRuntimeDirs) plus a behavioral
test; keep the execution_context path-classification assertion on update.md.
Re-point install.test's custom-config-dir assertion (kilo.jsonc/KILO_CONFIG)
to update-context.cjs where that detection now lives.

Full root suite: 2022 pass / 0 fail.

* fix(#498): CI — avoid bare gsd-tools in update.md; register new CLI modules

- update.md update-context invocation: resolve the PATH gsd-tools shim into a
  variable and call "$GSD_TOOLS" (never a bare `gsd-tools` command) — satisfies
  the #2851 workflow-bare-gsd-tools guard.
- Register package-identity.cjs and update-context.cjs in docs/INVENTORY.md
  (CLI Modules 76 -> 78 + rows) and regenerate docs/INVENTORY-MANIFEST.json,
  fixing inventory-counts and inventory-manifest-sync.

* test(#498): make update-context + parity tests OS-agnostic (Windows CI)

Two Windows-only test failures, both test-portability (production code is fine —
the real-fs CLI integration test passed on Windows):

- update-context resolver tests + bug-3608 behavioral test used POSIX path-string
  keys in their fake fs, but the resolver builds lookups via path.join/resolve
  (backslash + drive letter on Windows) → keys never matched → everything
  resolved to UNKNOWN/claude. Normalize fake-fs keys and gsdDir comparisons
  through path.resolve so they match on both platforms.
- package-identity parity test compared render() (LF) to the committed file,
  which Windows git checks out as CRLF (no .gitattributes eol rule). Normalize
  line endings before comparing, matching the repo convention
  (autonomous-decomposition, bug-3707).

* fix(#498): update.md backup must use GSD_DIR (adversarial-review finding)

The get_installed_version rewrite emits GSD_DIR but dropped the probe-loop
variables LOCAL_DIR/GLOBAL_DIR. The backup_custom_files step still read those,
so RUNTIME_DIR went empty for every LOCAL/GLOBAL install and detect-custom-files
was skipped — and since the update then runs a clean install that wipes managed
dirs (commands/gsd, get-shit-done), user-added files could be deleted without
the intended backup.

Set RUNTIME_DIR="$GSD_DIR" directly (the resolved config dir; empty for
UNKNOWN scope, which still skips the backup). Add a structural regression
(tests/issue-498-update-backup-runtime-dir.test.cjs).

* test(#503): re-point Antigravity .agent detection at the #498 projection

#499 moves the runtime/scope detection cascade out of update.md inline bash
into get-shit-done/bin/lib/update-context.cjs. The #503 regression test asserted
on the inline RUNTIME_DIRS array, which no longer exists, so it would fail
against the projected update.md even though the .agent guarantee is preserved.

Rewrite it to verify the surviving surfaces:
 - behavioral: resolveUpdateContext resolves a LOCAL ./.agent install to the
   antigravity runtime (the original root cause, now covered by adding
   ['antigravity', '.agent'] to the projection RUNTIME_DIRS table)
 - update.md prose classifier still maps /.agent/ -> antigravity
 - the post-update cache-clear for-dir loop still includes .agent

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#498): finish de-hardcoding consumers + close adversarial-review parity gaps

Restore the consumer de-hardcoding that is the point of the seam, and close the
parity gaps an adversarial review (codex) found in the update-context projection.

De-hardcode the repo slug + install command in the changeset tooling — #516
only single-sourced the package NAME, leaving 'open-gsd/get-shit-done-redux'
hardcoded in scripts/changeset/cli.cjs and github-release-notes.cjs. Route both
through the seam's repoSlug/packageName so a rename is a regenerate, not a hand
edit. The drift-lint real scan now reports zero divergent coordinate literals.

Projection parity vs the old inline bash, as ONE consistent rule
(trustedVersionAt) applied on every path:
 - expand a leading ~/ in preferredConfigDir before the fast path (the bash ran
   expand_home first; a custom --config-dir ~/foo otherwise fell to UNKNOWN)
 - trust a version only when BOTH VERSION and the update.md marker exist — fast
   path AND LOCAL/GLOBAL cascade; a partial dir falls to 0.0.0 keeping scope
 - apply the same same-path dedup to the 0.0.0 fallback so a partial install
   probed from cwd===home is not misdetected as LOCAL

Adds regression tests for tilde expansion, VERSION-only (cascade + fast path),
and the cwd===home partial-install dedup.

b0c5d86aba fix(#503): detect local Antigravity (.agent) installs in /gsd:update (#512)
* fix(#503): detect local Antigravity (.agent) installs in /gsd:update

The installer places local Antigravity installs in ./.agent/
(bin/install.js getDirName('antigravity') === '.agent'), but the
/gsd:update detection cascade in update.md only knew the global
Antigravity layout (.gemini/antigravity{,-ide,-cli}). A local .agent
install fell through to the `Otherwise -> claude` default, so the
update refreshed Claude artifacts instead of the Antigravity install.

Add `.agent` -> antigravity to all four runtime-dir surfaces in
update.md: the execution_context path classifier, the RUNTIME_DIRS
candidate array, the local-scope discovery loop, and the post-update
cache-clear loop (the last otherwise left a stale update indicator on
local Antigravity installs).

Note: the issue also cited update-context.cjs, which does not exist on
`next` (it is introduced by the still-open PR #499). The same .agent
fix should be carried into update-context.cjs when #499 lands.

48824f258c fix(#444): resolver preamble checks repo-local .claude install path (#476)
The gsd_run resolver preamble now probes
<repo-root>/.claude/get-shit-done/bin/${_GSD_SHIM_NAME} as the second
check — immediately after the existing get-shit-done/bin/ check and
before command -v / $HOME/.claude fallbacks. This covers the install
layout produced by npx @opengsd/get-shit-done-redux@latest --claude --local.

A _GSD_RUNTIME_ROOT variable is introduced to bind the repo-root
expression once and reuse it for both checks without repeating the
git rev-parse subshell.

76 workflow files regenerated via node scripts/sync-runtime-launcher.cjs.
All parity, size-budget, and new regression tests pass.

5ca646f015 feat(#443): unified cross-provider effort controls + fast-mode-aware routing (#463)
* test(#443): RED unified effort + fast_mode + resolve-execution

All 68 tests failing as expected — no implementation yet.
Covers: effort cascade (tier defaults, overrides, invalid fallthrough),
fast_mode cascade (boolean-only, tier defaults), resolveEffortForTier
escalation, renderEffortForRuntime clamping, resolve-execution CLI,
config schema new keys, QA hostile-input matrix.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* feat(#443): unified cross-provider effort + fast_mode knobs and resolve-execution query

Adds config-driven effort control (universal ladder: minimal<low<medium<high<xhigh<max)
and fast_mode propagation knobs, with per-runtime rendering that clamps the unique
tail values (max=Anthropic-only clamps to xhigh on Codex; minimal=Codex-only clamps
to low on Claude).

Key changes:
- config-schema.manifest.json: add effort.default, fast_mode.enabled as validKeys;
  add 4 dynamicKeyPatterns for effort.routing_tier_defaults, effort.agent_overrides,
  fast_mode.routing_tier_defaults, fast_mode.agent_overrides; fix stale _comment
- config-defaults.manifest.json: add effort and fast_mode blocks with tier defaults
- model-catalog.cjs: add EFFORT_RENDERING map, renderEffortForRuntime(), RUNTIMES_WITH_FAST_MODE
- model-profiles.cjs: re-export new catalog exports
- core.cjs: add resolveEffortInternal, resolveFastModeInternal, resolveEffortForTier,
  VALID_EFFORTS, EFFORT_SET, nextEffort; pass effort/fast_mode through loadConfig
- commands.cjs: replace reasoning_effort in cmdResolveModel with unified effort;
  add cmdResolveExecution (superset command with effort_rendered, effort_param,
  effort_propagation, fast_mode, fast_mode_supported)
- gsd-tools.cjs: add resolve-execution case with --effort/--fast-mode/--attempt flags
- tests/feat-443: 69 tests covering cascade, rendering, escalation, CLI, schema, QA matrix
- tests/commands.test.cjs: convert 3 reasoning_effort assertions to unified effort
- docs/CONFIGURATION.md: document effort + fast_mode + resolve-execution sections
- settings-advanced.md: list new effort/fast_mode keys in confirmation table

* refactor(#443): remove dead catalog effort lane; unify codex effort through renderEffortForRuntime

- Remove resolveReasoningEffortInternal (catalog-driven effort function) from
  core.cjs and its export; remove from commands.cjs destructure import
- Convert tests/issue-2517-runtime-aware-profiles.test.cjs: all 11 effort
  assertions now use resolveEffortInternal + renderEffortForRuntime; Claude
  effort is first-class (output_config.effort); unknown runtimes assert param===null
- Convert tests/feat-3023-model-phase-types.test.cjs: replace the entire
  resolveReasoningEffortInternal describe with unified effort assertions;
  effort derives from AGENT_DEFAULT_TIERS routing tier, not phase-type tier

* docs(#443): ADR for unified cross-provider effort + fast-mode routing

Co-Authored-By: Claude Opus 4.7 <noreply@anthropic.com>

* test(#443): architecture-level QA invariants + test-strategy doc

Add 48-test integration suite (feat-443-effort-fast-mode.integration.test.cjs)
covering 8 architectural invariants: cross-provider validity (never emit a value
the real API would 400 on), param/channel contract stability, resolve-execution
JSON contract (all 8 keys + correct types), totality across the full 33-agent
registry, fast-mode honesty (claude always fast_mode_supported=false), precedence
first-valid-wins matrix for both effort and fast_mode cascades, dynamic-routing
composition (effort escalation independent of model tier), and config-set round-trip
for all new effort/* and fast_mode/* key namespaces. Append test-strategy section
with invariant rationale and E2E gap documentation to docs/TESTING-SUITES.md.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test(#443): add failing install-wiring tests for effort per-runtime injection (RED)

TDD RED: 10 failing tests covering:
- Claude .md gets effort: injected per tier (planner=xhigh, mapper=low, executor=high)
- Gemini .md does NOT get effort: (already passing — Gemini-safe)
- Codex .toml gets model_reasoning_effort via unified resolver
- Config-driven: effort.agent_overrides drives both Claude .md and Codex .toml
- Source purity: agents/*.md have no effort: key (already passing)

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* feat(#443): wire effort per-runtime at install (Claude .md frontmatter + Codex .toml unified)

- Import AGENT_DEFAULT_TIERS and renderEffortForRuntime from model-catalog.cjs
- Add readGsdEffectiveEffortConfig(targetDir): reads merged effort config from
  .planning/config.json (per-project wins) + ~/.gsd/defaults.json (global fallback),
  same probe pattern as readGsdRuntimeProfileResolver
- Add resolveInstallTimeEffort(effortCfg, agentName): pure function matching
  resolveEffortInternal() precedence (agent_overrides > routing_tier_defaults > default > 'high')
  without loadConfig side-effects (no sub-repo detection, no migration writes)
- Claude agent copy loop: inject `effort: <value>` into frontmatter ONLY for
  runtime === 'claude'; all other .md runtimes (Gemini, Qwen, Hermes, etc.) stay
  effort-free (Gemini-safe source contract preserved in agents/*.md)
- generateCodexAgentToml: add effortCfg param; emit model_reasoning_effort from
  unified resolver (replaces old catalog entry.reasoning_effort); Codex clamps
  max → xhigh via renderEffortForRuntime('codex', ...)
- installCodexConfig: pass readGsdEffectiveEffortConfig(targetDir) to
  generateCodexAgentToml so per-project config wins for Codex .toml too
- Update failing tests to GREEN: 12/12 pass; all 17 install tests pass;
  2847/2848 unit tests pass (1 pre-existing failure: policy-shell-pinning)

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* refactor(#443): source install effort defaults from manifest (kill drift) + guard test

Replace hardcoded _GSD_EFFORT_MANIFEST_TIER_DEFAULTS and the 'high' fallback in
resolveInstallTimeEffort with values read from config-defaults.manifest.json at
module init, using the same __dirname-relative path install.js already uses for
all shared manifests. Add feat-443-effort-defaults-drift.test.cjs to assert
equality between install.js's runtime constants and the manifest on every CI run.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#443): reconcile Codex TOML tests with unified effort design

The #443 unified effort resolver makes generateCodexAgentToml always emit
model_reasoning_effort (driven by resolveInstallTimeEffort, not model_profile_overrides).
The test 'generated TOML omits reasoning_effort when runtime has none' had an
obsolete premise — model_profile_overrides.reasoning_effort:'' no longer suppresses
unified effort. Convert it to assert the new invariant: Codex TOML always carries a
valid model_reasoning_effort from the agent's routing tier (xhigh for gsd-planner,
a heavy-tier agent), while model_profile_overrides model override is still respected.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#443): make install.js effort resolution lazy (no load-time side effects breaking launcher-parity)

Replace module-load-time IIFE + hard throw (config-defaults.manifest.json read)
and top-level require of model-catalog.cjs with a lazy _getGsdEffortCatalog()
getter that initialises on first call from resolveInstallTimeEffort /
generateCodexAgentToml / Claude .md effort injection. Requiring install.js in
unrelated test contexts (e.g. runtime-launcher-parity) no longer triggers
manifest IO or throws, eliminating the load-time side effect that changed
subprocess exit codes / stderr on the bench.

Drift-guard exports (_GSD_EFFORT_MANIFEST_TIER_DEFAULTS / _GSD_EFFORT_MANIFEST_DEFAULT)
preserved as lazy getter properties on module.exports so feat-443-effort-defaults-drift
still validates them without forcing eager load.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#443): isolate install-wiring test HOME to stop \$HOME/.claude pollution breaking launcher-parity

runGlobalInstall() now redirects HOME to a per-call isolated tmpdir in addition
to the existing runtime-specific env-var redirects (CLAUDE_CONFIG_DIR,
GEMINI_CONFIG_DIR, CODEX_HOME). This ensures install.js code that uses
os.homedir() directly — including the ~/.cache/gsd update-check deletion,
~/.gsd/defaults.json reads, and any HOME-relative npm subprocess writes —
never touches the real \$HOME during the test.

Without the HOME isolation the install test (which is new to this branch and
is now picked up by Docker's raw \`tests/*.test.cjs\` glob) could write or
delete files under the real \$HOME, causing runtime-launcher-parity test (D)
to fail: (D) asserts a loud non-zero exit when \$RUNTIME_DIR/gsd-tools.cjs is
absent and gsd-tools is not on PATH, but the launcher's \$HOME/.claude fallback
arm succeeds if \$HOME/.claude/get-shit-done/bin/gsd-tools.cjs exists.

Also sets GSD_SKIP_STALE_SDK_CHECK=1 to suppress the \`npm ls -g\` subprocess
that the global installer spawns — irrelevant to effort-wiring assertions,
slow, and potentially writes to ~/.npm cache.

All 12 feat-443 install-wiring assertions preserved. Drift-guard 5/5. Unit
suite 2848/2850 (pre-existing policy-shell-pinning.test.cjs failure on next).

* chore(#443): add changeset fragment for effort + fast-mode routing

Co-Authored-By: Claude Opus 4.7 <noreply@anthropic.com>

* fix(#443): set GSD_TEST_MODE before requiring install.js in drift-guard test to prevent HOME leak

Without GSD_TEST_MODE=1, require('bin/install.js') runs the module's main
install block (guarded by !GSD_TEST_MODE), performing a real global Claude
install into $HOME/.claude/. On CI ubuntu where node is on standard PATH,
the launcher's $HOME/.claude fallback arm then finds gsd-tools.cjs, causing
runtime-launcher-parity test (D) to exit zero when it must exit non-zero.

Root cause: feat-443-effort-defaults-drift.test.cjs (unit suite) runs
alphabetically before runtime-launcher-parity.test.cjs in the same node
--test invocation. Each runs in a separate worker process but shares the
same HOME. The drift test's install leaks gsd-tools.cjs into that HOME,
then the launcher test's bash subprocess finds it via the $HOME/.claude arm.

Fix: add process.env.GSD_TEST_MODE = '1' at the top of the drift-guard
test, before the require(installPath) call. This matches the pattern used
by feat-443-effort-fast-mode.test.cjs and feat-443-effort-install-wiring
.install.test.cjs.

* fix(#443): deterministic resolve-execution arg parsing + validate install-time effort (Codex adversarial findings)

Finding 1: resolve-execution --effort low gsd-planner misrouted 'low' as the agent.
Replace find(non-dash) with a proper flag-consuming loop that collects a single
positional; validate missing/extra positionals and malformed --attempt values.

Finding 2: resolveInstallTimeEffort returned unvalidated effort strings (e.g. "ultra")
verbatim. Each precedence layer now checks GSD_EFFORT_SET (imported once from
core.cjs) before accepting a value, mirroring resolveEffortInternal exactly.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(#443): newline-agnostic effort frontmatter injection (Windows CRLF) + CRLF-safe assertions

Extracts injectEffortFrontmatter(content, effortValue) pure helper that detects
EOL (LF vs CRLF) from the opening '---' line and inserts 'effort: <value>'
before the closing '---' delimiter using the same EOL as the surrounding
frontmatter. Regex now uses /^---\r?\n([\s\S]*?)^---\r?$/m instead of the
LF-only /^(---\n[\s\S]*?)(---)(\n|$)/ that silently skipped CRLF files on
Windows (git core.autocrlf=true checkout).

Also adds 7 unit tests covering LF, CRLF, idempotency, no-frontmatter, and
complex frontmatter cases. Exports injectEffortFrontmatter from module.exports.

Fixes 6 CI failures in tests/feat-443-effort-install-wiring.install.test.cjs
on windows-latest runners (lines 138, 145, 152, 261, 345, 356).

af36416d14 fix(#160): resume partially-executed phases before current_phase routing (#406)
When a session dies mid-execution (hang, token exhaustion, API drop),
STATE.md's current_phase can be advanced past a phase that still has
PLAN.md files without matching SUMMARY.md files. Without this fix,
/gsd-next and /gsd-progress would route by current_phase and silently
skip the partially-executed phase, producing a data-loss-shape outcome.

Adds a Route 0 cross-phase incomplete-execution scan to both
next.md and progress.md. Before any current_phase-based routing,
the scan finds the lowest-numbered phase where plans outnumber
summaries and routes to /gsd-execute-phase <that-phase> to resume it.
Opt out with --no-resume to fall back to the prior-phase defer prompt;
--force bypasses all gates as before.

Rework (codex review):
- Route 0 now ordered AFTER Gates 1-3 (repo/state validity always run)
  but BEFORE the prior-phase completeness-scan defer prompt — eliminating
  the double-decision where the default path would both prompt the user
  (C/S/F) AND resume the phase anyway. Prior-phase defer prompt moved to
  a new prior_phase_completeness step; only reached via --no-resume.
- --force flow made coherent across all three steps: safety_gates jumps
  directly to determine_next_action, skipping Gates, Route 0, AND
  prior_phase_completeness. resume_incomplete_phase and prior_phase_completeness
  now correctly state --force never reaches them. success_criteria entry
  updated to reflect --force → determine_next_action (not prior_phase_completeness).
- Scan uses $GSD_SDK (canonical resolver form) throughout next.md, matching
  the file's existing convention. progress.md uses $ROADMAP already loaded
  by analyze_roadmap. Neither file uses bare gsd-sdk.
- Errors are surfaced rather than suppressed: removed 2>/dev/null on the
  main roadmap.analyze call; added explicit WARNING emission when the scan
  cannot run, so the invariant fails closed instead of failing open.
- Predicate aligned to plans-without-summaries (plans.length > summaries.length)
  in both files, consistent with determine_next_action Route 4.
- command references use canonical /gsd: namespace form

* fix(#373): replace unquoted $GSD_SDK with space-safe gsd_run launcher

Workflow bash blocks resolved the runtime as GSD_SDK="node $GSD_TOOLS" and
invoked it unquoted ($GSD_SDK query ...). On install paths containing spaces
(e.g. /Volumes/Mini Me/...) the unquoted expansion word-split into
`node /Volumes/Mini gsd-tools.cjs ...`, failing with "Cannot find module
'/Volumes/Mini'" and getting masked by `2>/dev/null || echo "{}"` into a
silent empty state.

Replace the string variable with a single-line shell launcher that defines a
gsd_run function, invokes the runtime with a fully-quoted path and "$@", and
preserves the local-cjs / installed-gsd-tools-on-PATH fallback (#3668) plus
the loud not-found error and install hint. The launcher uses _GSD_SHIM_NAME
indirection so no workflow emits the /gsd-tools substring that the do.md
dispatcher-parity scanner would misread, and is single-line to stay within
the per-file progressive-disclosure line budgets (#2551).

The canonical launcher lives in
get-shit-done/workflows/_runtime-launcher.snippet.sh, is propagated once per
file by scripts/sync-runtime-launcher.cjs, and is locked by
tests/runtime-launcher-parity.test.cjs (fails CI on drift, on a reappearing
$GSD_SDK token, or on a /gsd-tools substring). Dependent workflow-assertion
tests are updated from $GSD_SDK to gsd_run, and the runtime launcher is
registered in CONTEXT.md.

Fixes #373

* ci(#373): add changeset fragment and allow-test-rule for parity guard

The runtime-launcher parity test is a structural drift guard that reads
workflow markdown to assert the canonical launcher is present and the
retired $GSD_SDK / /gsd-tools tokens are absent; annotate it with
allow-test-rule per the no-source-grep lint escape hatch. Add the required
.changeset fragment for this user-facing fix.

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* test(#373): make parity PATH-fallback assertion cross-platform (Windows)

Subtest (E) compared GSD_TOOLS against the Node-side absolute temp path,
but the value originates from git-bash which reports the POSIX form, so the
prefix comparison failed on windows-latest while the launcher itself worked
(the installed stub was invoked). Assert the resolved binary by normalized
suffix (/bin/gsd-tools, not .cjs) instead of the absolute prefix; the
behavioral stub-invocation assertion is unchanged.

85c67b4b21 chore: invalidate bug-2543 outdated namespace invariant + document /gsd-<cmd> migration in CONTEXT.md (#164)
* test(meta): invalidate bug-2543 outdated /gsd:<cmd> namespace invariant

The "no /gsd-<cmd> hyphen form in source files" scan (test 2 of the
describe block) is skipped via test.skip. Motivation:

- Bug-3584 (2026-05-15) introduced runtime-slash.cjs, which intentionally
  emits `/gsd-${token}` for skills-based runtimes. This is the correct
  canonical form for runtime-persisted strings (ROADMAP.md, STATE.md,
  recommended_actions, fix hints).
- Bug-2543 was last updated 2026-05-12, three days BEFORE bug-3584 landed.
  The "no hyphen form" invariant was never updated to reflect the new
  two-tier model.
- PR #154 first-pass: an agent misread bug-2543 and reverted the correct
  `/gsd-plan-phase` to `/gsd:plan-phase` in phase-lifecycle-policy.ts:156,
  breaking tests/bug-3584-runtime-slash-emitters.test.cjs. A 2nd-pass agent
  reverted.

The canonical active invariant for runtime-emitter context is:
  tests/bug-3584-runtime-slash-emitters.test.cjs

The remaining 4 tests in this file (commands/gsd/ existence, command
filename slug format, transformer behavior, non-command identifier safety)
are still valid and remain active — only test 2 is skipped. This avoids
vacuous-truth (a full describe-block skip would let all assertions pass
trivially).

See CONTEXT.md § "Slash-command form: /gsd-<cmd> vs /gsd:<cmd>" (added
in the companion commit) for the full two-tier model documentation.

User directive: 2026-05-23 session authorizing this invalidation.

* docs(context): document /gsd-<cmd> slash-command migration for AI agents

Adds CONTEXT.md § "Slash-command form: /gsd-<cmd> (current) vs /gsd:<cmd>
(legacy)" to give future agents an unambiguous reference for which form
to use in which context.

Covers:
- The two-tier model (source text → colon; runtime-emitted strings → hyphen)
- The runtime-emitter authority: get-shit-done/bin/lib/runtime-slash.cjs
- The canonical invariant test: bug-3584-runtime-slash-emitters.test.cjs
- The PR #154 incident: how an agent misread bug-2543 and applied the wrong form
- An explicit "Context for AI agents" block: stop and re-read if bug-2543 is
  influencing a patch
- A note that DEFECT.AGENT-RETIRED-SLASH-SYNTAX-DRIFT.* predicates (written
  pre-two-tier) are stale for runtime-emitter contexts

Also updates the stale DEFECT predicates at the end of CONTEXT.md with a
clarifying note (the predicates remain for historical reference; the new §
supersedes their fix-forward guidance for runtime-emitter contexts).

Motivation: PR #154 first-pass incident (2026-05-23 session).
Canonical runtime contract: tests/bug-3584-runtime-slash-emitters.test.cjs.
Companion: test(meta) commit invalidating bug-2543's scan.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test(meta): re-activate bug-2543 scan as scoped invariant, exclude runtime-emitter contexts

Codex adversarial review of PR #164 [high finding]: the single content-scanning
test in bug-2543 was test.skip, leaving a vacuous test surface. The transformer/
filename unit tests remained active but no live source surface was guarded.

Fix: replace test.skip with an active scoped invariant.

Scope change:
- REMOVED get-shit-done/bin/lib/ from SEARCH_DIRS entirely. That directory is
  runtime-emitter territory (runtime-slash.cjs, *.generated.cjs,
  phase-lifecycle-policy.ts) and intentionally uses /gsd-<cmd> (hyphen) per
  bug-3584's contract. Scanning it causes false positives that led to PR #154
  first-pass incident.
- Added RUNTIME_EMITTER_EXCLUDES set documenting exactly why each file is exempt.
- Remaining SEARCH_DIRS (workflows/, references/, templates/, commands/gsd/,
  agents/, hooks/) are Claude-facing source — colon form is correct there.

Verified: 9/9 tests pass, 0 skipped.
Canonical runtime-emitter contract: tests/bug-3584-runtime-slash-emitters.test.cjs
bug-2543's scoped scope: excludes bin/lib/ (runtime-emitter contexts).

* docs(context): rewrite slash-command section as directory-level matrix

Codex adversarial review of PR #164 [high finding]: the previous CONTEXT.md
section was internally contradictory — line 616 claimed /gsd-<cmd> was globally
canonical while lines 634/640 correctly stated Claude-facing source text uses
/gsd:<cmd>. Same document; opposite claims.

Root cause: the first push of this section (2026-05-23) overstated the hyphen
form as universal, when the project has always had a two-tier model.

Fix: replace the section with an unambiguous directory-level matrix.

New structure:
- Single table mapping each directory/surface to its correct form and enforcement.
- "How to choose" decision tree (4 steps, replaces ambiguous prose).
- "What was WRONG previously" retains historical motivation and adds the
  PR #164 contradiction incident to the record.
- "Context for AI agents" updated: ban mass-rewrites based on single test failure,
  cite the two-tier model explicitly.

Two-tier model (unchanged from reality, now clearly documented):
- Claude-facing source (commands/, agents/, workflows/, etc.): /gsd:<cmd> colon.
- Runtime-emitter contexts (runtime-slash.cjs, *.generated.cjs, ROADMAP.md
  persistence): /gsd-<cmd> hyphen per bug-3584 invariant.

Canonical authorities: bug-2543 (colon contract), bug-3584 (hyphen contract).

* fix(workflows): replace dead /gsd-* tokens with live registry forms

Codex adversarial review of PR #164 [medium finding]: stale slash-command
references in user-facing workflow content. Registry-backed sweep confirmed
the following tokens are not in commands/gsd/ registry.

Tokens removed/updated (dead token → live registry form):

1. /gsd-remove-workspace → /gsd:workspace --remove <name>
   File: get-shit-done/workflows/list-workspaces.md:58
   Reason: no commands/gsd/remove-workspace.md; remove-workspace is a subcommand
   of /gsd:workspace (commands/gsd/workspace.md, --remove flag).

2. /gsd-list-workspaces → /gsd:workspace --list
   File: get-shit-done/workflows/remove-workspace.md:35
   Reason: no commands/gsd/list-workspaces.md; list-workspaces is a subcommand
   of /gsd:workspace (commands/gsd/workspace.md, --list flag).

3. /gsd-list-phase-assumptions <phase> → /gsd:discuss-phase <phase> --assumptions
   File: get-shit-done/workflows/list-phase-assumptions.md:17-18 (usage block)
   Reason: no commands/gsd/list-phase-assumptions.md; the workflow is invoked
   via commands/gsd/discuss-phase.md with --assumptions flag.

4. /gsd-list-phase-assumptions 2 → /gsd:discuss-phase 2 --assumptions
   File: get-shit-done/references/continuation-format.md:59
   Reason: same as #3.

Sweep scope: user-facing markdown only (workflows/, references/). Runtime-emitter
hyphen-form references in bin/lib/ are guarded by bug-3584 and were not touched.

* chore(164): add changeset fragment for adversarial-review fixes

The 3 commits addressing Codex review touched user-facing surfaces
(get-shit-done/workflows/, get-shit-done/references/, CONTEXT.md,
bug-2543 test). Add fragment to satisfy changeset-lint.

d8b432da1e fix(#14): wire --auto flag through progress→next handoff (#148)
* fix(#14): document --auto in progress.md and wire chaining logic in next.md

The --auto flag was accepted by /gsd:progress --next --auto but silently
ignored: it was not documented in the <flags> section of progress.md and
had no handling in the next.md show_and_execute step, so it was dropped
at the handoff boundary and never produced step chaining.

- Add --auto and --next --auto entries to progress.md <flags>
- Update progress.md <process> to explicitly list --auto as a passthrough arg
- Add --auto chaining logic to next.md show_and_execute: after each step
  completes, re-invoke /gsd:progress --next --auto until milestone complete
  or a blocking decision is required
- Add regression test (4 assertions) covering all three fix points

* fix(#14): bump lint-test-file-count progress ceiling for bug-14 test

bug-14-progress-auto-flag-dropped.test.cjs resolves to the "progress"

* chore(#14): add changeset fragment

* fix(#14): address review feedback — differentiate duplicate tests, scope assertions to specific blocks

Test 2 now extracts the <process> block and asserts --auto within it,
distinguishing it from test 1's <flags>-level check. Remaining assertions
use semantic token matches (--auto, --next --auto) that are robust to
benign reformatting.

334a64168e chore(npm): rebrand packages to @opengsd scope (#127)
* chore(npm): rebrand packages to @opengsd scope

Rename:
- get-shit-done-redux → @opengsd/get-shit-done-redux
- @gsd-redux/sdk → @opengsd/gsd-sdk

Add publishConfig.access=public for first-time scoped publish.
CLI binary names (get-shit-done-redux, gsd-sdk, gsd-tools) unchanged.

Sweeps install commands, npx invocations, CI publish/version-check
workflows, tests, docs, READMEs (all translations), and the
PACKAGE_NAME constant in check-latest-version.

Bumps qs 6.15.1 → 6.15.2 to clear a moderate advisory surfaced by
the audit-clean test (GHSA-q8mj-m7cp-5q26).

Closes #126

* chore: pin 2.0.0 release + remove canary workflow

- Bump both packages 1.50.0-canary.0 → 2.0.0 for first @opengsd publish
- Remove .github/workflows/canary.yml and canary dist-tag handling in
  release.yml / release-sdk.yml
- Drop canary section from VERSIONING.md

Refs #126

* chore: address review findings + harden tarball-smoke timeout

- .changeset/opengsd-org-rename.md: match project's custom
  parse.cjs frontmatter (type: Changed / pr: 127); the scoped
  @changesets/cli keys were silently rejected.
- CONTEXT.md: drop two canary-stream policy lines and a dangling
  DEFECT.CANARY-VERSION-LEAK.cross-ref now that canary.yml is gone.
- tests/release-tarball-smoke.install.test.cjs: pass
  timeout: 600_000 for npm pack + global install; the 3-minute
  runNpm default was timing out on slower Docker hosts (cartographer).

Refs #126

* fix(sdk): add missing type/runtime devDependencies for build

prepublishOnly invokes tsc which couldn't resolve @types/node,
@types/ws, or synckit. They had been hoisted from root but were
not declared in sdk/'s own package.json — first publish from a
clean SDK tree failed.

* fix(ci): use npm pack stdout instead of glob to find tarball

`npm pack --silent` for a scoped package (@opengsd/get-shit-done-redux)
produces `opengsd-get-shit-done-redux-*.tgz`, not `get-shit-done-redux-*.tgz`.
Capture the filename from stdout instead of a hardcoded glob so the step
works regardless of package name format.

Fixes smoke (ubuntu-latest, 22, false) CI failure.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* ci: treat workflow-file changes as test-skip eligible

`.github/workflows/install-smoke.yml` (and other workflow files)
were in neither `test.yml` paths nor `test-skip.yml` paths-ignore,
so neither workflow ran on a workflow-only commit — leaving the
required test-skip check perpetually missing.

Refs #126

* chore: reset version to 1.0.0 for first @opengsd publish

Nothing has been published yet under the @opengsd scope, so the
inaugural release uses 1.0.0 rather than 2.0.0. The "major bump"
in the changeset reflects the breaking install-command change for
users migrating from the prior unscoped `get-shit-done-redux`, not
a numeric continuation from a 1.x line under the new identity.

7ad1a5edf5 fix(3668): isolate --local install from global gsd-sdk (#89)
* fix(3668): isolate --local install from global gsd-sdk

- `buildGsdSdkVersionMismatchReport` now accepts `opts.isLocal`; when
  true it sets `fix_command` to `npx get-shit-done-cc@latest --claude
  --local` instead of `npm install -g …`, removing the misleading global
  upgrade suggestion for local installs.
- Propagate `isLocal` from `installSdkIfNeeded` into the mismatch report
  builder so the right fix_command reaches the renderer.
- Export `buildGsdSdkVersionMismatchReport` and
  `renderGsdSdkVersionMismatchReport` so tests can assert on the IR
  contract directly.
- Add `command -v gsd-sdk … elif node "$GSD_TOOLS"` preflight SDK
  resolution block to all 69 workflow files that called bare `gsd-sdk`
  with no fallback, matching the pattern established in update.md,
  execute-phase.md, and quick.md.
- Add `tests/bug-3668-local-install-sdk-soft-dep.test.cjs` with 5 tests
  covering Defects 1-3, including a CI lint guard that blocks future
  workflow regressions.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* changeset: add Fixed entry for #3668

* fix(3668): add allow-test-rule to suppress false lint-no-source-grep violation

The test reads workflow .md files (product content) to assert structural
invariants — not .cjs source files. The file-presence check is the only
viable IR for markdown guard patterns. Add the // allow-test-rule annotation
so lint-no-source-grep passes.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3668): fix do.md false-positive and discuss-phase.md size overflow

Two CI failures introduced by the 69-workflow preflight block:

1. do.md: the path `bin/gsd-tools.cjs` contains `/gsd-tools` which the
   bug-2954 parity test regex `/\/gsd[:-]([a-z][a-z0-9-]*)/g` mistakenly
   extracts as a slash command named `tools`. Fix: store the shim filename
   in _GSD_SHIM_NAME so the path construction no longer contains a static
   `/gsd-tools` literal. Also wire $GSD_SDK into the actual query call.

2. discuss-phase.md: the file was at 499 lines (the 500-line budget from
   #2551). Adding the 11-line preflight block pushed it to 510, failing
   workflow-size-budget.test.cjs. Fix: compress the 11-line preflight +
   2-line invocations into 3 lines (one-liner guard + two $GSD_SDK calls)
   returning the file to 499 lines while retaining the command -v guard
   required by bug-3668-local-install-sdk-soft-dep.test.cjs.

* fix(3668): wire \$GSD_SDK through all workflow callsites (#3797)

PR #3797 introduced the resolution preflight block (setting \$GSD_SDK) in
69 workflows but left every downstream gsd-sdk callsite using the bare
command. On local-only installs the preflight exits cleanly, then the
very next line fails with 'command not found'. This is the structural
gap the Codex review flagged.

Changes:
- 687 bare `gsd-sdk` callsites replaced with `\$GSD_SDK` across 75
  workflow files (all bash/sh fenced blocks excluding the resolution
  guard blocks themselves)
- execute-phase.md: was missing the preflight block entirely — added
  the standard 11-line resolution block at the initialize step
- execute-phase.md: inline `if command -v gsd-sdk` availability guard
  (legacy #3384 fallback) replaced with `\$GSD_SDK` + error fallback
  since the new preflight guarantees SDK availability or exits 1
- 6 sub-workflow files (discuss-phase/modes/*, execute-phase/steps/*)
  that have no preflight of their own but use \$GSD_SDK — these are
  loaded by parent workflows that set the variable; callsites updated
  to use \$GSD_SDK so they work when variable is in scope

Transformation script used: /private/tmp/fw2.js (regex-based fence
parser with segment join invariant verification — preserves all blank
lines and prose formatting).

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test(3668): upgrade CI guard to detect bare callsite routing (#3797)

The previous Defect 3 test checked that 'command -v gsd-sdk' appeared
as a string in the file — a guard-presence check, not a callsite-routing
check. A workflow with the preflight block but 40 bare gsd-sdk calls
below it passed the old test. This is exactly the bug state PR #3797
was supposed to fix.

Upgraded test:
- Parses each workflow file into markdown segments using a regex-based
  fence extractor (preserves all content invariantly)
- Skips bash/sh blocks that contain 'command -v gsd-sdk' (those are
  resolution guards — bare references there are expected)
- Flags any remaining bash/sh block line that invokes gsd-sdk without
  the \$ prefix (isBareGsdSdkInvocation predicate)
- Counter-test proves the predicate correctly flags real callsite lines
  and correctly exempts guard assignments, comments, and \$GSD_SDK refs

Also adds helper functions parseMarkdownSegments, isBareGsdSdkInvocation,
and findMdFiles which are used by both the upgraded Defect 3 test and
the counter-test.

This test would have caught the originally-shipped bug: the preflight
block was present but callsites still used bare gsd-sdk.

* fix(tests): update workflow content tests to accept \$GSD_SDK callsite form (#3797)

Six regression tests assert on the exact textual pattern of gsd-sdk calls
inside workflow .md files. After the #3797 callsite replacement (687 bare
`gsd-sdk` invocations replaced with `\$GSD_SDK`), these tests failed because
they searched for the literal string `gsd-sdk query <cmd>` which no longer
appears at callsites.

Updated each test to accept both the pre-#3797 bare form and the post-#3797
variable form using `(?:\$GSD_SDK|gsd-sdk)` regex alternation (or two-branch
`includes()` checks for non-regex assertions). The structural invariants each
test enforces are unchanged — we're accepting the same behavioral contract
through the new callsite surface.

Tests fixed:
- bug-2334-quick-gsd-sdk-preflight: find init.quick call via \$GSD_SDK or bare
- bug-2661-roadmap-sync-parallel: roadmap.update-plan-progress call pattern
- bug-3360-codex-execute-phase-worktrees: RUNTIME config-get call detection
- bug-3381-verify-work-workstream: init.verify-work / phase.mvp-mode calls
- enh-2433-todo-phase-linking: commit call in new-milestone.md
- enh-2792-namespace-skills: validate.context invocation in context_check step

* fix(tests): update remaining workflow content tests to accept \$GSD_SDK form (#3797)

After #3797 callsite replacement, ultraplan-phase.test.cjs and worktree-cleanup.test.cjs
still assert bare gsd-sdk form. Update to accept either \$GSD_SDK or gsd-sdk. Also trim
the execute-phase.md preflight comment to stay within the XL line-count budget (1810).

* fix(3668): adopt inline-per-fence SDK resolution + restore safety semantics

The brief offered three options:
  (a) inline preflight block per fence
  (b) wrapper script
  (c) shared shell fragment sourced at the top

71 of 72 workflow files already had inline preflight blocks (just broken ones).
Option (b)/(c) would have required changes to install.js + a new shared artifact,
with significant risk of breaking the install pipeline. Option (a) was the path
of least resistance and least new blast radius.

**BLOCKER 1+2+3 (quick.md — GSD_SDK never assigned):**
- quick.md had 12 `$GSD_SDK` references but zero `GSD_SDK=` assignments.
- Added proper local-first preflight block with `git rev-parse --show-toplevel`
  path (not the broken `CLAUDE_FILE_PATHS` which is always empty in Claude Code).
- Each Bash fence in Claude Code runs as a fresh `bash -c`, so env vars don't
  persist. The preflight block must appear in every fence that uses $GSD_SDK.

**BLOCKER 4 (execute-phase.md — || exit 1 dropped):**
- Restored `|| exit 1` after every `worktree.cleanup-wave` call. SDK safety
  refusals (drift detection #3174, deletion block #2384) must surface, not be
  swallowed by the old `|| { fallback }` branch.

**F5 (verify-work.md untyped fence):**
- Changed bare `gsd-sdk` in an untyped fence to `$GSD_SDK`.
- Changed fence tag from untyped to `bash`.

**F6 (non-recursive readdirSync):**
- Defect 2 test now uses `findMdFiles` (recursive) to cover workflow
  subdirectories, not the flat `fs.readdirSync` that missed subdirs.

**F7 (lint misses untyped fences):**
- `parseMarkdownSegments` now treats `lang === ''` fences as bash-fences.

**F8 (missing propagation test):**
- Added two propagation tests in the Defect 3 describe block.

**F9 (priority inverted — global before local):**
- All 72 workflow files now check `[ -f "$GSD_TOOLS" ]` before `command -v gsd-sdk`.
- Path: `$(git rev-parse --show-toplevel 2>/dev/null || pwd)/get-shit-done/bin/gsd-tools.cjs`

**F10/F11 (broken quoting):**
- Changed `GSD_SDK="node "$GSD_TOOLS""` → `GSD_SDK="node $GSD_TOOLS"` across all files.

**SDK-absence fallback removal:**
- The old `|| { STATE_BACKUP=...; while IFS=...WAS_DELETED...; done }` fallback
  code was dead — preflight now exits if neither local nor global SDK exists.
  Removed from quick.md, execute-phase.md. Tests updated to verify SDK delegation
  rather than inline shell mechanics.

**Tests updated:**
- bug-2384, bug-2501, bug-2838, bug-3091, bug-3195, bug-3521, bug-3668,
  worktree-cleanup — all updated to reflect SDK delegation contract.
- Defect 2 test now uses bash-fence scan (not raw content) to skip docs-only
  gsd-sdk prose references (e.g. discuss-phase/modes/text.md).

Closes #3668

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3668): restore _GSD_SHIM_NAME indirection in do.md to prevent false-positive

The top commit re-introduced a literal /get-shit-done/bin/gsd-tools.cjs path
in do.md, causing bug-2954 test to match /gsd-tools as an unshipped slash
command. Restore the _GSD_SHIM_NAME variable indirection (from ff9939e5) to
break the literal path while preserving local-first preference order.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(tests): update worktree.test.cjs to accept SDK delegation contract (#3797)

Mirror the contract update already applied to worktree-cleanup.test.cjs:
- pre-merge deletion check tests: accept worktree.cleanup-wave + deletion
  mention as valid (inline --diff-filter=D was in the removed shell fallback)
- quick.md bug-2431 tests (lock-aware, unlock retry, residual warning): accept
  worktree.cleanup-wave delegation as sufficient (these safety behaviors are
  now handled internally by the SDK cleanup-wave command)

execute-phase.md tests unchanged: it retains inline .git/worktrees/, locked,
git worktree unlock, and Residual worktree in its cleanup-tail snippet.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test(3668): refactor bug-2384 and bug-2838 from grep to structured assertions

Replace content.includes() on readFileSync-bound variables with parser
functions that split lines and return typed boolean fields, matching the
project's no-source-grep contract (lint-no-source-grep rule F/G).

2a915c1b82 chore: migrate references from gsd-build to open-gsd/get-shit-done-redux (#120) (#121)
Security-motivated migration of all stale repository and npm-scope references.

Three categories of changes (58 files, 174 substitutions):

1. gsd-build → open-gsd (security-critical):
   - .github/workflows/release-sdk.yml — npm token comment, tarball filename pattern
   - .github/workflows/hotfix.yml — same
   - .changeset/fix-3406-detect-stale-sdk-shadow.md — @gsd-build/sdk → @open-gsd/sdk
   - .changeset/sharp-quails-leap.md — same
   - get-shit-done/workflows/update.md — CHANGELOG raw GitHub URL

2. GSD-redux org slug → open-gsd (canonical rename):
   - package.json + sdk/package.json — repository/homepage/bugs metadata
   - All README.*.md — live badge and link sections
   - CONTRIBUTING.md, CONTEXT.md, QUICK-WINS-CONFIRMED-BUGS.md
   - .coderabbit.yaml, .release-monitor.sh, scripts/sync-rulesets.sh
   - docs/** — all live agent/ADR/user-facing documentation
   - tests/** — repo slug assertions and test fixtures
   - scripts/changeset/cli.cjs + github-release-notes.cjs
   - .github/ISSUE_TEMPLATE/*, .github/pull_request_template.md
   - bin/install.js, get-shit-done/bin/lib/model-catalog.cjs
   - sdk/HANDOVER-*.md, sdk/src/*.test.ts

3. CLAUDE.md (gitignored local file — not in this commit):
   Updated separately outside git: --repo gsd-build/get-shit-done →
   --repo open-gsd/get-shit-done-redux with security warning.

Intentionally unchanged: CHANGELOG.md, docs/RELEASE-*.md,
.changeset/README.md, .changeset/build-hooks-atomic-write.md,
README.md migration table (historical fork record),
tests/changeset-serialize.test.cjs line 78 (serialization fixture).

The gsd-build/get-shit-done repo is compromised (rug-pull documented in
README.md). Do not push to or interact with that repo.

ea67479bfb fix(3496): include all version patterns in changelog extraction (#90)
* fix(3496): include all version patterns in changelog extraction

parseChangelog now handles multi-line bullets (continuation lines
starting with two or more spaces) where the (#NNNN) PR trailer
appears on a continuation line, not the opening dash line. The
previous single-line regex silently dropped every such bullet,
causing Feature/Enhancement sections to return 0 entries.

Also adds an `extract` subcommand to scripts/changeset/cli.cjs:
  changeset/cli.cjs extract --from VERSION --to VERSION [--changelog FILE] [--json]
Extracts releases strictly after --from (exclusive) and up to and
including --to (inclusive). Accepts v-prefixed versions. Exits 2
when no releases fall in range, giving /gsd:update a deterministic
range-aware helper instead of vague/manual extraction.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test(3496): use production parseChangelog in markdown-mode assertion

Replace raw stdout.includes() in the emits-markdown test with a
parseChangelog call on the output so the assertion targets version
strings via the production parser rather than a raw substring match.
Eliminates the output-grep anti-pattern flagged by test-rigor.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* chore(changeset): add fragment for fix #3796 (issue #3496)

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3496): reject malformed --from/--to semver in extract with structured error

`parseSemver` coerced non-numeric components to 0 (e.g. `1.41.x` → `1.41.0`),
making range selection silently wrong under typos or version-shape drift.

Add a strict N.N.N validation gate before comparison; exit 1 with a JSON
error report when either bound fails. Add two regression tests covering
alphabetic and dotted-letter inputs.

Codex adversarial review finding: high severity (scripts/changeset/cli.cjs:226-244)

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3496): preserve bullets without PR trailer in parseChangelog (pr: null)

Previously flushBullet() silently discarded any bullet that lacked a
trailing (# NNNN) token. On the real CHANGELOG.md this dropped 7 entries
from v1.41.0 alone, so cmdExtract returned incomplete release notes to the
/gsd:update confirmation step.

Store PR-less bullets as { body, pr: null } instead. Update cmdExtract's
textOutput renderer to emit `- body` (no trailer) for null-pr bullets.

Add regression tests:
  - serialize: preserves bullets without trailer as pr:null (not dropped)
  - cli extract: preserves PR-less and PR bullets together in extracted JSON
  - cli extract: rejects malformed --from/--to (1.41.x, foo) with exit 1

Codex adversarial review finding: high severity (scripts/changeset/serialize.cjs:64-73)

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3496): wire extract into update.md + reject pre-release in range, fix CHANGELOG parser edge cases

BLOCKER fixes:
- F1: workflows/update.md show_changes_and_confirm step now invokes
  `scripts/changeset/cli.cjs extract --from $INSTALLED_VERSION --to
  $LATEST_VERSION --changelog $CHANGELOG_TMP --json` with explicit exit-2
  handling ("no releases in range") and fallback text. The prior prose
  ("extract entries between versions") was never wired to the binary and
  silently skipped intermediate versions (#3496).
- F2: releases.filter in cmdExtract now rejects any rel.version that does
  not pass SEMVER_RE before numeric-tuple comparison.  parseSemver('1.0.0-rc.1')
  previously returned [1,0,0] (same as '1.0.0'), causing pre-release entries to
  corrupt range queries. Architectural choice: skip pre-release + 4-part
  versions with a stderr warning; full semver §11 pre-release ordering deferred
  to a consolidation issue (see F8 note below).

MAJOR fixes:
- F3 (serialize.cjs): releaseMatch regex updated to
  /^##\s+\[([^\]]+)\](?:\([^)]*\))?\s*(?:-\s*(\S+))?/ so linked-header
  format `## [1.42.1](url) - 2026-05-15` captures the date correctly.
- F4 (serialize.cjs): continuation-line test now checks `!/^\s+-\s/`
  so ` - nested item` terminates the current bullet instead of folding in.
- F5 (cli.cjs): 4-part versions (e.g. 1.0.0.1) fail SEMVER_RE and are
  skipped by the same guard added for F2. No separate code path needed.
- F6 (serialize.cjs): v-prefix stripped from in-file version capture;
  `## [v1.0.0]` now parses as version "1.0.0".
- F7 (serialize.cjs): continuation-line indentation relaxed from /^[ \t]{2}/
  to /^\s+/ so 1-space-indented continuations fold correctly (F4's
  bullet-terminator guard prevents nested bullets from being folded).

MINOR fixes:
- F9 (cli.cjs): unknown-command path now exits 1 instead of 2 (exit 2
  is reserved for "no releases in range" semantic).
- F10 (cli.cjs): text-mode exit-2 path now writes
  "no releases found in range (from=X, to=Y)" to stderr.
- F11 (tests): exit-2 test now asserts r.json is present and
  r.json.releases.length === 0.

NOTE — F8 (semver consolidation): this codebase has 5+ distinct semver
comparators with divergent pre-release policies; this PR adds a 6th (the
SEMVER_RE guard in cmdExtract). A follow-up consolidation issue should be
filed to unify all call sites. Out of scope for this PR.

Regression tests added for F1–F6: pre-release exclusion, linked-header
date parsing, nested-bullet termination, 4-part/v-prefix edge cases, and
workflow wiring. All 15 tests pass.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(lint): add allow-test-rule annotation to F1 workflow wiring test

The F1 test reads get-shit-done/workflows/update.md (a product markdown
file, not CJS source) to assert the extract subcommand invocation was
wired. The lint-no-source-grep detector flags any readFileSync-bound
variable used with .includes() regardless of file extension; annotate
with // allow-test-rule to exempt this legitimate product-content
assertion.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test: apply deterministic barrier to locking-bugs #1927 config-set test

The 'both concurrent config-set calls persist their values' test used
Promise.all([execAsync(A), execAsync(B)]) without a barrier, which is
non-deterministic under Docker load: one subprocess can complete before
the other starts (no real contention) or both can race O_EXCL and observe
stale fs state (lost write / assertion failure).

Mirrors the locking-bugs:180 and :235 redesigns: erect a barrier file,
spawn both subprocesses, wait for both to signal readiness via ready files,
then drop the barrier simultaneously so both config-set calls genuinely
contend on withPlanningLock.

74cb493373 fix(3784): expose adaptive in model_profile settings flow (#91)
* fix(3784): expose adaptive in model_profile settings flow

Split the single 4-option model-profile AskUserQuestion into a two-question
flow: Q1 (Adaptive / Standard tier / Inherit) routes top-level intent; Q2
(Quality / Balanced / Budget) appears only when Standard tier is chosen.
Updates the confirm table and success_criteria to include adaptive.

Adds regression test asserting all five valid profiles are reachable
interactively via the settings UI.

* changeset: add Fixed entry for #3784 / PR #3795

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3784): correct Q2-skip comment and remove duplicate brace in settings.md

Codex review followup:
- Replaced vague "preserve existing config" comment with accurate description:
  Q1 still writes model_profile on Adaptive/Inherit branches; only Q2 is skipped.
- Removed stray duplicate `{` line before the Spawn Plan Researcher question block
  (pseudocode had two consecutive `{` openers, one spurious).

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3784): address review — gate Q2 structurally, define cancel rule, harden tests

Addresses gsd-code-reviewer (M1/M2/m1/m2/m3/m4) and codex adversarial
(Q2 gating, save-mapping, Claude-only wording, step-of-2 wording).

- F1: Replace //comment-only Q2 gating with Conditional visibility block
  (mirrors code_review_depth / graphify.auto_update structural pattern)
- F2: Define model_profile cancel rule in update_config step (leave
  existing value unchanged when Q1="Standard tier…" but Q2 cancelled)
- F3: Fix Adaptive description — remove "Claude only" tail; describe
  heavy/light role tiers across all supported runtimes
- F4: Remove "step 1 of 2 for standard profiles" from Q1 question text
  (2-step nature now structurally documented by Conditional visibility)
- F5: Fix vacuously-true test disjunct (|| content.includes('Adaptive')
  always true — 6+ occurrences); assertion now requires role-based cost
  optimization + heavy roles wording
- F6: Add 4-option cap enforcement test (ASK_USER_QUESTION_OPTION_CAP=4
  named constant, counts per question object not per AskUserQuestion call)
  and brace-balance regression test (guards against bd53925f recurrence)

* docs(3784): list adaptive in model_profile reference docs

dcacf3eea9 fix(3805): schema-aware log_to_state row appending in fast.md (#85)
* fix(3805): schema-aware log_to_state row appending in fast.md

REPRO: fast.md log_to_state unconditionally echoed a hardcoded 4-cell
row (| date | fast | task | ✅ |) into STATE.md. When quick.md Step 7
had already created the "Quick Tasks Completed" table with 5 columns
(| # | Description | Date | Commit | Directory |), fast.md appended a
malformed 4-cell row → broken Markdown table.

FIX: fast.md log_to_state now reads the existing table header, counts
columns, and checks for the expected column names from quick.md Step 7.
If the 5-column schema is confirmed, it appends a properly-formed 5-cell
row. If the schema is unrecognized, it skips the write with a warning
rather than corrupt the table.

Pattern source: quick.md Step 7 (schema-aware matching).

ANTI-PATTERN SWEEP: Only fast.md and quick.md contain direct STATE.md
table writes in workflows/. quick.md Step 7 is already schema-aware.
No other workflow candidates found.

a7abc6df2f fix(3657): skip false-fail when pristine hash drifts after GSD update (#3767)
* test(3657): RED+shape-lock for verify-reapply-patches pristine-drift

Adds tests/bug-3657-verify-reapply-patches-pristine-drift.test.cjs:
- Core regression: exits 0 with reason=OK_PRISTINE_DRIFT_DETECTED when
  on-disk gsd-pristine/ hash does not match backup-meta.json.pristine_hashes
- Counter-tests: real FAIL_USER_LINES_MISSING still caught when hashes match;
  over-broad mode unchanged when backup-meta.json is absent; clean run
  reports 0 failures when everything matches
- Multi-file: drift + real-failure handled independently per file

Updates tests/bug-2969-verify-reapply-patches.test.cjs REASON shape-lock to
include OK_PRISTINE_DRIFT_DETECTED (added by the #3657 fix).

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3657): verify-reapply-patches skips pristine when hash drifts

When gsd-pristine/ is refreshed to a newer GSD version after a backup is
captured, the on-disk pristine's SHA-256 no longer matches the hash recorded
in backup-meta.json.pristine_hashes. Using the wrong-version pristine as the
diff baseline inverts the delta: every line the upstream removed between the
two versions appears as a "user-added line that must survive", producing
spurious FAIL_USER_LINES_MISSING false positives (Bug #3657).

Fix:
- Add sha256() and readPristineHashes() helpers to verify-reapply-patches.cjs
- In verifyFile(), when a pristine_hashes entry exists for the file, compare
  the on-disk pristine's SHA-256 against it before accepting the baseline
- On hash mismatch, return immediately with status=ok and the new
  REASON.OK_PRISTINE_DRIFT_DETECTED code, skipping the diff rather than
  false-failing
- When no pristine_hashes entry exists (older installer / absent backup-meta),
  fall through to the pre-fix behaviour (use on-disk pristine as-is)
- Export sha256, readPristineHashes, and OK_PRISTINE_DRIFT_DETECTED

New REASON code OK_PRISTINE_DRIFT_DETECTED is added to the frozen enum.
Exit code contract is unchanged: 0 for gate pass (including skipped-due-drift
files), 1 for real user-content failures, 2 for structural errors.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* chore(3657): update changeset to reference PR #3767

* fix(3657): surface drifted_files in verify-reapply-patches JSON report

Extend the top-level JSON report shape with two additive fields:
  - `drifted: N`  — count of files skipped due to pristine-snapshot drift
  - `drifted_files: [...]`  — relative paths of those files

Per-file shape is unchanged (status:'ok' + reason:OK_PRISTINE_DRIFT_DETECTED)
for backward compat. Drift still exits 0; `failures` count is unaffected.

This gives workflow Step 5a structured data to gate on so drifted files are
no longer silently treated as a full PASS (codex adversarial-review finding 1).

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3657): reapply-patches workflow halts on drifted files instead of silent pass

Insert a "Step 5a: drift check" block between the exit-code check and the
failures check in workflows/reapply-patches.md Step 5a. The new block:

  1. Parses `drifted` + `drifted_files` from the JSON report (added in the
     companion prod-code commit).
  2. When DRIFTED_COUNT > 0, emits a formatted HALT message naming each
     drifted file and instructs the user to re-baseline before re-running.
  3. Sets DRIFT_DETECTED=true and exits non-zero so subsequent steps cannot
     execute while drift is unresolved.

Drift is a distinct third state: it is not a failure (no missing user lines
were proven) but it is also not a clean pass (the diff was skipped entirely).
Existing pass/fail logic for VERIFY_STATUS and failures count is unchanged.

Closes the silent-skip gap identified in codex adversarial-review finding 1.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test(3657): assert drifted_files report shape + workflow Step 5a drift check

Finding 1 (BLOCKER) — three new tests in bug-3657 test file:
  - Single drifted file: drifted=1, drifted_files contains the file path,
    failures=0, per-file shape unchanged (backward compat).
  - Multi-file drift: drifted=2, drifted_files lists both paths, clean file
    absent from array.
  - No-drift baseline: drifted=0, drifted_files=[] always present in output.

Finding 2 (WARNING) — structural test on workflow source:
  - Asserts Step 5a contains "Step 5a: drift check" heading.
  - Asserts DRIFTED_COUNT, drifted_files, and DRIFT_DETECTED are referenced
    (confirming the gate exists and uses the structured report fields).
  - Asserts drift-check block appears before VERIFY_STATUS check (exit-code
    is 0 for drift, so the drift check must precede the non-zero gate).

Also updates bug-2969 shape-lock to include the two new additive fields
(drifted, drifted_files) per the contract change in the prod-code commit.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3657): address pr-review-toolkit + codex review + CI failures

- lint-tests: add // allow-test-rule: source-text-is-the-product at file
  top of bug-3657 test file; the inline comment at line 477 was a prose
  sentence, not a file-level annotation, so the lint scanner did not
  recognise it as the bypass token
- Windows test failures (4 subtests): normalize relPath to forward slashes
  before pristineHashes key lookup in verifyFile(); on Windows path.join
  produces backslash-separated relPath values but backup-meta.json stores
  keys with forward slashes, causing the hash lookup to silently return
  undefined, falling through to use-as-is mode and producing the same
  false FAIL_USER_LINES_MISSING that the fix was meant to prevent

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(tests): bump verify allowlist ceiling 9→10 for bug-3657 test file

Adding tests/bug-3657-verify-reapply-patches-pristine-drift.test.cjs in
the previous commit pushed the verify module from 9 to 10 test files.
The lint-test-file-count gate (added in #6313baad on main) enforces that
modules cannot exceed their allowlist ceiling, so all 6 test platforms
plus lint-tests and coverage failed with:

  FAIL_EXCEEDS_ALLOWLIST: verify count=10 ceiling=9

The fix is to raise the ceiling from 9 to 10 and set issue=3767.
This file does not exist on this branch yet (introduced on main after
the branch diverged) so we add it here. The merged CI state will see
the bumped ceiling and pass.

86e067924f fix(3727): wire --fix flag dispatch in code-review workflow (#3743)
* test(3727): add failing test for --fix flag dispatch in code-review workflow

Adds bug-3727-code-review-fix-flag-dispatch.test.cjs with:
- Pure-function tests on parseCodeReviewFlags() / resolveCodeReviewWorkflow()
  from new code-review-flags.cjs typed IR module
- Structural docs-parity tests asserting dispatch_fix step exists in
  code-review.md and that initialize step references code-review-flags.cjs

Structural tests FAIL today (RED): workflow has no dispatch_fix step and
no reference to code-review-flags.cjs in initialize. IR-level tests pass
because the lib module is introduced in this commit as the typed seam.

Fixes #3727

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3727): wire --fix/--all/--auto flags through code-review workflow

The initialize step now parses --fix, --all, and --auto from argv (via the
code-review-flags parser seam added in the previous test commit) and the
workflow dispatches gsd-code-fixer when --fix is truthy and the review
returned findings. Fixes the regression introduced when PR #2947 closed
#2946 without actually shipping the workflow dispatch.

Fixes #3727

* chore(3727): update changeset to reference PR #3743

The fix commit landed with the linked-issue number as the pr: field
placeholder. Updating to the actual PR number now that the PR is open.

* fix(3727): register code-review-flags.cjs in INVENTORY.md and manifest

Add missing row for get-shit-done/bin/lib/code-review-flags.cjs to the CLI

docs/INVENTORY-MANIFEST.json to fix three failing CI tests:
  - cli-modules-doc-parity: every CLI module must have a row in INVENTORY.md
  - inventory-counts: headline "CLI Modules (N shipped)" must match file count
  - inventory-manifest-sync: INVENTORY-MANIFEST.json must match filesystem

e690f1bc90 fix(3718): shell-free Node.js UI safety gate — fixes PowerShell silent-fail (#3718)
* fix(workflows): add word-boundary anchoring to UI safety gate grep

Replace unanchored grep -iE "UI |..." alternation with POSIX ERE
word-boundary-anchored form:

  LC_ALL=C grep -iE "(^|[^[:alnum:]])(UI|...)([^[:alnum:]]|$)"

Unanchored form matched 'ui' inside 'requirements', 'view' inside
'overview' and 'review', 'form' inside 'performance'/'platform'/
'transform' — producing HAS_UI=0 on 100% of standard roadmap phases
(every phase contains a **Requirements**: field).

Fix applied to both plan-phase.md:625 and autonomous.md:284.
LC_ALL=C added for POSIX locale portability on both BSD and GNU grep.

Closes #3706

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* test(workflows): add regression tests for UI safety gate false-positives (#3706)

- bug-3706-ui-safety-gate-false-positives.test.cjs: 36-test suite covering
  both plan-phase.md and autonomous.md gate behavior; verifies that
  Requirements/overview/performance/platform/transform/review/build/screening
  do NOT trigger the gate, while standalone UI/view/form/screen/dashboard/
  component/lowercase-ui/hyphenated-non-UI DO trigger it.

- autonomous-ui-steps.test.cjs: update stale assertion that checked for the
  old broken grep pattern; now asserts the word-boundary-anchored form.

Test strategy: extract the POSIX ERE pattern from the workflow file and
simulate grep match semantics in JS (no shell exec, no source-grep).

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* chore(changeset): add Fixed fragment for PR #3718 (UI safety gate false-positives)

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(workflows): document compound-token boundary contract; add comment to gate

Addresses adversarial review finding: word-boundary anchoring intentionally
does not match tokens embedded in compound alphanumeric words (e.g.
"microfrontend", "dashboardWidget", "uiSpec"). This is correct behavior —
gsd-roadmapper generates natural English prose, not camelCase compounds.
Hyphenated forms ("micro-frontend") and spaced forms are caught by the
anchored pattern (hyphen is [^[:alnum:]]).

Add inline comment in both workflow files explaining the pattern intent,
the false-positive prevention, and the compound-word contract.

Add 4 tests (2 per workflow) documenting the compound-word contract:
- "microfrontend" (compound) must NOT trigger gate (documented behavior)
- "micro-frontend" (hyphenated) MUST trigger gate (correct true-positive)

* fix(3718): replace shell grep gate with shell-free Node.js helper

Moves UI safety gate logic from `LC_ALL=C grep -iE` (silently broken on
Windows PowerShell — locale env-var prefix not recognised by pwsh) to
`bin/lib/ui-safety-gate.cjs` (Node.js, reads via stdin to avoid ARG_MAX).

Path is anchored via `git rev-parse --show-toplevel` (GSD_REPO_ROOT) to
avoid CWD-sensitive failure when Claude Code executes from a subdirectory.

Word-boundary regex is identical to the original POSIX ERE pattern:
  (^|[^a-zA-Z0-9])(TOKEN)([^a-zA-Z0-9]|$)
Exit codes mirror grep: 0 = UI found, 1 = not found.

Tests: 51/51 pass — includes spawnSync shell:false + stdin cross-shell
portability tests and ARG_MAX large-input test.

* fix(3706): address pr-review-toolkit + codex review findings

- Multi-token-per-line: use matchAll to capture all UI tokens
- Add test for multiple distinct tokens on same line
- Clarify ASCII vs POSIX [:alnum:] in word-boundary comment
- Correct misleading "path anchored" comment in plan-phase/autonomous workflows
- Remove UI_GATE_PATTERN from module.exports (internal implementation detail)

* fix(state): restore ACQUIRE_LOCK_RETRY_ERRNOS in acquireStateLock (#3718)

Commit 473c279c removed ACQUIRE_LOCK_RETRY_ERRNOS and replaced the
correct `throw err` path with `return lockPath`, which silently
"succeeds" on any non-EEXIST error — allowing two concurrent processes
to both hold the lock simultaneously and causing lost updates.

This restores the set of recoverable transient errno codes (Docker
overlay-fs EINVAL/EIO/ENOENT, NFS ESTALE, POSIX EAGAIN/EINTR, Windows
EPERM/EBUSY) that should retry, and restores `throw err` for genuinely
fatal codes. Equivalent to commit 47983914 on main.

3d52f5ee46 refactor(tests): consolidate Init Command Module — 7 files → 5 (#3756)
* fix(3687): update insert-phase docs and roadmapper to use --insert flag

Updates stale references in insert-phase.md workflow and
gsd-roadmapper.md agent to use the consolidated /gsd:phase --insert
command syntax instead of the retired /gsd-insert-phase and
/gsd:phase insert forms.

Closes #3687

* refactor(tests): consolidate Init Command Module — 7 files → 5

Closes #3755

Merges `tests/init-manager-deps.test.cjs` (#2267 regression) into
`tests/init-manager.test.cjs` (718 LOC), and
`sdk/src/query/init-progress-precedence.test.ts` (#2674 regression)
into `sdk/src/query/init-complex.test.ts` (788 LOC).

The 800 LOC ceiling prevents further consolidation:
- `tests/init.test.cjs` is pre-existing at 1630 LOC
- `sdk/src/query/init.test.ts` is at 791 LOC
- `sdk/src/query/init-workstream-milestone-op.test.ts` is a distinct
  seam testing initMilestoneOp, roadmapAnalyze, and
  resolveQueryRuntimeContext workstream resolution.

Also adds Init Command Module Glossary entry to CONTEXT.md.

Allowlist update deferred to rebase after #3738 merges (allowlist
file does not exist on origin/main).

* fix(init): initExecutePhase preserves same-milestone archived phase dir (#3469)

When `phases clear` archives current-milestone phases into
`.planning/milestones/<version>-phases/` but the workflow is still on
that same milestone, `shouldDropArchivedPhaseMatch` was unconditionally
dropping the archived dir match. This caused `phase_dir: null` when the
phase was still executing in the current milestone.

Fix: detect when `phaseInfo.archived === currentMilestone` (read from
STATE.md) and skip the drop. The #2391 regression guard is safe because
that scenario involves archived.version != current milestone.

Also corrects two tests in `initRemoveWorkspace` to expect thrown
GSDError instead of `{ data: { error } }` — the production code was
intentionally changed to throw for CLI non-zero exit propagation.

ca2644a71a fix(worktree): unlock-retry on locked cleanup + startup orphan sweep (#3707) (#3719)
* fix(worktree): unlock-retry on locked cleanup + startup orphan sweep (#3707)

Two root causes fixed:

1. **In-session cleanup blocked**: `executeWorktreeWaveCleanupPlan` now attempts
   `git worktree unlock <path>` then retries `git worktree remove --force` when the
   initial single-force remove fails on a locked worktree. Previously every cleanup
   after a successful merge was silently blocked.

2. **Cross-session orphan accumulation**: new `reapOrphanWorktrees` helper sweeps
   `.git/worktrees/*/locked` at startup. It reaps entries where the pid is dead,
   the branch tip is an ancestor of the default branch (ancestry guard prevents data
   loss on squash-merge repos), and the lock mtime is older than 5 minutes (race
   guard). Wired into `quick.md` and `execute-phase.md` startup blocks guarded by
   `USE_WORKTREES != false`.

SDK: adds `worktree.reap-orphans` query command (routes through gsd-tools.cjs).
Tests: 11 real-fs tests covering unlock-retry, dead-pid reap, live-pid skip,
unmerged skip, fresh-mtime skip, idempotent double-call, and structural wiring.

* chore(changeset): add Fixed fragment for PR #3707 (worktree orphan cleanup)

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(worktree): fix test portability on Windows + macOS for bug-3707 reap tests

- worktreeMeta helper: replace /\/\.git$/ with /[/\\]\.git$/ so the
  gitdir path suffix is stripped on both Windows (backslash) and Unix.
- worktreeMeta helper: normalize CRLF→LF before splitting porcelain
  blocks, fixing block parsing when git emits CRLF on Windows.
- reapOrphanWorktrees: replace single 'main' rev-parse with a
  [defaultBranch, 'main', 'master'] candidate loop so test fixtures
  without a remote origin (where branch may be 'master') don't bail
  early. Intentionally excludes 'HEAD' to prevent false reaping when
  HEAD is detached or on a feature branch (Codex adversarial finding).

* fix(worktree): CI green — macOS symlink path, Windows test helper, pid portability, EPERM liveness

Four fixes to get macOS + Windows CI from red to green:

1. **macOS symlink mismatch** (worktree-safety.cjs): `reapOrphanWorktrees` now
   builds a canonical→listed path map from `git worktree list --porcelain` using
   `fs.realpathSync.native`. Uses the listed path (as git knows it) for
   `git worktree unlock/remove`, not the gitdir-derived path. Fixes the
   `/var/folders` vs `/private/var/folders` discrepancy on GitHub macOS runners
   where `git worktree unlock <realpath>` was silently failing because git's
   list stored the unresolved symlink path.

2. **Windows path separator in test helper** (test file): `worktreeMeta`
   `.replace(/\/\.git$/, '')` → `.replace(/[/\\]\.git$/, '')`. On Windows,
   git writes backslash separators in the gitdir file; the Unix-only regex was
   causing `Cannot find .git/worktrees/<name>` for all Suite 2 tests.

3. **Non-portable PID in tests** (test file): All `'999999'` dead-PID literals
   replaced with `deadPid()` helper that spawns a real short-lived child, captures
   its PID, and returns it after exit. Eliminates flakiness on Linux systems where
   `pid_max` can reach 4194304, making 999999 a live PID.

4. **EPERM fail-closed in isPidAlive** (worktree-safety.cjs): `catch { return false }`
   → checks `err.code === 'EPERM'` and returns `true` (alive). On Windows and
   cross-user scenarios, `process.kill(pid, 0)` throws EPERM for live but
   inaccessible processes; treating that as dead would reap a live worktree.

Adversarial review via codex confirmed:
- Squash-merge repos: fail-closed (CONCERN, not BUG — by design, not data-loss)
- canonicalToListed map: SAFE (fail-closed on realpathSync error)
- Concurrent reapers: SAFE (both prune; second gets skipped: remove_failed)
- Startup blocking: CONCERN (no global cap, 10s/call × N worktrees) — tracked,
  not fixed here (requires separate perf work)
- gsd-sdk missing: SAFE (quick.md checks and fails fast with guidance)

All 27 local tests + Docker (holodeck) green.

* fix(worktree): address codex adversarial findings — fail-closed default branch + CRLF map

Two fixes from codex adversarial review of PR 3718:

1. **Default branch resolution (data-loss risk)**: `reapOrphanWorktrees` now
   uses `refs/remotes/origin/<branch>` exclusively when a remote is configured.
   If `origin/HEAD` is absent but a remote exists, we bail out (fail-closed)
   rather than falling back to a local `main`/`master` that may not be the
   real integration branch. The `main`/`master` fallback is only used when
   there is provably no remote (local-only test fixtures).

2. **CRLF normalization in canonical-path mapper**: The `worktree list
   --porcelain` output was split on '\n\n' without normalizing CRLF first.
   On Windows, git emits CRLF, which caused block-splitting to fail and
   left the canonicalToListed map only partially populated, weakening the
   symlink/path-mismatch fix introduced earlier.

3. **Windows 8.3 short-path fix (test helper)**: Both `beforeEach` blocks
   now call `resolvedTmpDir()` which pre-resolves `os.tmpdir()` via
   `fs.realpathSync.native` so temp paths avoid RUNNER~1-style short names
   that git stores in long form, causing worktreeMeta path comparisons to
   fail on Windows CI.

All 11 real-fs + 16 unit tests green locally.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(worktree): adversarial findings + macOS CI path-mismatch fix

## Root cause (macOS CI fail)
`reapOrphanWorktrees` stored `worktreePath` (gitdir-derived, real path via
git's symlink resolution, e.g. `/private/var/folders/…`) in results, while
the test's `wtDir` used the unresolved symlink form (`/var/folders/…`). After
reaping, `canonicalPath(wtDir)` can no longer call `realpathSync.native`
(directory gone), so it falls back to `path.resolve` — which returns the
symlink form — causing the `result.find()` comparison to miss.

## Fixes applied

### Source — worktree-safety.cjs
1. **Finding 1 (fail-closed PID check)**: Non-parseable lock content (e.g.
   `"Locked by claude-code agent-xxx"`) is now treated as ALIVE with reason
   `lock_owner_unknown`, not as dead. Previously it fell through as dead.
2. **Finding 1b (EPERM safe)**: `isPidAlive` call wrapped in try/catch; any
   thrown error (EPERM = process exists but cross-user on Windows) → ALIVE.
3. **Finding 2 (startup warning)**: `cmdWorktreeReapOrphans` now writes a
   one-line stderr warning when ≥1 entry is skipped or when reaper throws,
   while keeping exit-zero so workflows don't break.
4. **Finding 3 (default-branch discovery)**: Local-only fallback now tries
   `init.defaultBranch` config and HEAD symref before `main`/`master`, so
   repos configured with `trunk`, `dev`, etc. get correct orphan detection.
5. **macOS path fix**: Result entry for reaped worktrees now uses `gitKnownPath`
   (from `git worktree list`) instead of `worktreePath` (from gitdir file),
   ensuring the caller always sees the path git uses for the worktree.

### Test — bug-3707-locked-worktree-cleanup.test.cjs
6. **macOS CI fix**: Pre-compute `wtDirCanonical = canonicalPath(wtDir)` before
   calling `reapOrphanWorktrees` so the comparison works after removal.
7. **Gap 1**: New test — Claude Code lock format (`"Locked by claude-code …"`)
   must not be reaped; asserts `status=skipped, reason=lock_owner_unknown`.
8. **Gap 2**: New test — `isPidAlive` throwing EPERM → must not reap.
9. **Gap 3**: New test — repo with `init.defaultBranch=trunk`; merged worktree
   must be reaped (verifies trunk is discovered as the integration branch).

* fix(test): raise waitForStoppedAt timeout 2 s → 5 s for Windows/Node22 CI load

Subprocess write latency exceeds 2 s on loaded windows-latest/Node22 runners
(test duration was 6181 ms); 5 s gives sufficient headroom without changing
any production behaviour.

6a5fa59129 feat(3081): auto-trim review prompts for small-context model reviewers (#3708)
* feat(3081): auto-trim review prompts for small-context model reviewers

Adds review.max_prompt_tokens and review.max_prompt_tokens_per_reviewer
config keys. When configured, the /gsd-review workflow deterministically
trims the assembled prompt before sending to each reviewer (drop CONTEXT
→ RESEARCH → REQUIREMENTS; head-shrink PROJECT.md; tail-truncate PLANs
proportionally; reserve disclosure-note tokens upfront). Trim metadata
is recorded in REVIEWS.md frontmatter. Reviewer is skipped with a
warning if even the minimum review set exceeds the budget.

Closes #3081

* fix(3081): register prompt-budget in SDK query registry and update inventory manifest

review.md references `gsd-sdk query prompt-budget` at three call sites, but the
command had no handler in the SDK registry — failing the registry-integration
drift-guard test on all 6 CI matrix legs. Added a native TypeScript SDK handler
(sdk/src/query/prompt-budget.ts) that ports the applyBudget logic from the CJS
module, registered it in DOMAIN_STATIC_CATALOG, and regenerated
docs/INVENTORY-MANIFEST.json to include the new cli_modules/prompt-budget.cjs entry.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3081): bump ws to 8.20.1 and allowlist prompt-budget sibling pair

Two additional CI failures after the registry fix:

1. ws moderate CVE (GHSA-58qx-3vcg-4xpx, uninitialized memory disclosure):
   The advisory covers ws >=8.0.0 <8.20.1. Both root and sdk/package.json
   pinned ^8.20.0 which resolved to 8.20.0. Bumped both to 8.20.1 to clear
   the npm audit drift-guard test (bug-3588-npm-audit-clean.test.cjs).

2. lint-shared-module-handsync detected the new prompt-budget.ts / prompt-budget.cjs
   sibling pair without an allowlist entry. Added a cooperatingSiblings entry
   to scripts/shared-module-handsync-allowlist.json with classification and
   justification matching the established pattern.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3081): align prompt-budget skip semantics across CJS and SDK dispatch paths

Replace brittle `[ $EXIT -eq 2 ]` guards with `[ $EXIT -ne 0 ]` in all three
local-reviewer blocks (Ollama, LM Studio, llama.cpp) in workflows/review.md.
Any non-zero exit from prompt-budget now triggers a skip with a descriptive
warning — exit 2/11 prints "budget too small", any other non-zero prints
"unexpected exit code". This ensures the SDK bridge dispatch path (exit 11
via GSDError(Blocked)) triggers the same skip as the CJS path (exit 2).

The SDK handler (sdk/src/query/prompt-budget.ts) already writes both metadata
and prompt files before throwing, so no change needed there.

The Ollama block also gains the missing OLLAMA_SKIP guard so the reviewer
invocation is actually skipped (previously the block only suppressed the
OLLAMA_PROMPT_FILE update but still ran the curl invocation).

SDK integration path (hardFailed via GSDError(Blocked) → exit 11) is covered
by handler unit tests in tests/prompt-budget.test.cjs; no gsd-sdk-*.test.cjs
exercising the full bridge dispatch for this command exists yet — that gap
remains and is documented here.

* fix prompt-budget trim ordering and review guard follow-ups

* perf: optimize prompt-budget and dedup reviewer trim workflow

* fix(3708): drop source-grep theater tests to satisfy lint-no-source-grep

All four test files added in commit 2df566ed were pure source-grep theater:
they read .cjs / .ts / .md source files and asserted that specific string
literals were present or absent. None exercised runtime behaviour.

Deleted:
- tests/gsd-tools-memory-optimizer.test.cjs   — 7 includes() on gsd-tools.cjs
- tests/prompt-budget-hotpath-optimizer.test.cjs — includes() on prompt-budget.cjs + .ts
- tests/prompt-budget-io-optimizer.test.cjs   — includes() on prompt-budget.ts + gsd-tools.cjs
- tests/review-workflow-budget-dedup.test.cjs — includes() on review.md

Behavioural coverage for the prompt-budget feature already exists in
tests/prompt-budget.test.cjs and tests/prompt-budget-cli.test.cjs (also
added by this PR). No replacement tests needed.

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(3708): correct budget-pressure threshold and minSet accounting

Two bugs in applyBudget caused premature trimming and false hard-fails:

1. UNNEEDED_TRIM: budgetUnderPressure compared baseTokens against
   effectiveBudget - NOTE_RESERVE_TOKENS, triggering trim pressure 80
   tokens before the budget was actually exceeded. Fix: compare against
   effectiveBudget directly; NOTE_RESERVE_TOKENS are still reserved in
   contentBudget once real pressure is confirmed.

2. FALSE_HARDFAIL: minSet included NOTE_RESERVE_TOKENS unconditionally,
   treating the note as mandatory even when no trim would occur and no
   note would be injected. Fix: exclude NOTE_RESERVE_TOKENS from minSet;
   a prompt that fits untrimmed needs no note and must not hard-fail.

Both fixes applied in CJS and TypeScript implementations. Two regression
tests added (cycles 11 and 12) that reproduce each case behaviorally.

b01089c05a fix(#3689): use find instead of chained ls for continue-here scan (#3693)
* fix(#3689): use find instead of chained ls for continue-here scan

The check_incomplete_work step in resume-project.md chained six bare-glob
ls arguments to discover .planning/.continue-here*.md handoff files.
Under zsh's default NOMATCH option (macOS default shell), the first
non-matching glob aborts the entire command during word-expansion,
silently dropping every pattern after it — including
.planning/.continue-here*.md, which holds the canonical pause checkpoint
for default-context handoffs. The 2>/dev/null || true guard suppresses
ls's own stderr / exit code but has no effect on the shell's pre-exec
glob-abort.

Replace the chain with two find invocations:
  find .planning -maxdepth 3 -name '.continue-here*.md' -print 2>/dev/null
  find . -maxdepth 1 -name '.continue-here*.md' -print 2>/dev/null

find does not use shell glob expansion and tolerates absent directories
on both bash and zsh.

Adds tests/bug-3689-resume-glob-nomatch.test.cjs covering:
  - zsh -o nomatch with .planning/.continue-here-AT-1234.md present and
    no spike/sketch/deliberation subdirs (the regression scenario)
  - bash default, same layout
  - zsh -o nomatch with no checkpoints anywhere (clean exit, no output)
  - text invariant: resume-project.md no longer carries the chained-ls
    pattern and does carry the find-based scan

Closes #3689

* fix(#3689): guard find commands with || true for Windows safety

Restore the trailing '|| true' guard that the original chained ls had,
per the windows-robustness.test.cjs invariant (informational bash
commands in critical workflows must not let an exit-1 from find on
exotic platforms tank the resume-project.md flow).

* fix(#3689): address CodeRabbit review feedback

- Set changeset frontmatter pr: 3693 (was 3690 placeholder); the release-
  note link now points at the right PR.
- Use createTempDir / cleanup from tests/helpers.cjs instead of local
  tmpdir/cleanup wrappers, per repo test standards.

* fix(#3689): update bug-3446 discovery contract to new find-based scan

bug-3446 enforced three text invariants on the chained-ls implementation
that this PR replaces with find. Update the assertions to verify the
same three discovery paths are still covered:

- .planning/.continue-here*.md at depth 1 -> find .planning -maxdepth 3
- .planning/sketches/SKETCH-NNN/.continue-here*.md at depth 3 -> same
  find with -maxdepth >= 3 (assertion now reads the actual depth and
  enforces a lower bound so future changes can deepen but not shallow it)
- repo-root .continue-here*.md legacy fallback -> find . -maxdepth 1

The discovery contract is preserved; only the implementation under
inspection changes.

* test(#3689): convert bug-3446 from source-grep to behavioral assertions

CodeRabbit flagged the text-regex assertions on the workflow source as a
violation of the no-source-grep testing standard. Replace them with a
behavioral integration test that:

  1. Extracts the actual check_incomplete_work bash block from
     resume-project.md (so the test stays in sync with whatever the
     workflow does, no string match required).
  2. Plants three handoff files in a temp dir covering the three
     discovery surfaces bug #3446 originally filed:
     - .planning/.continue-here.md (depth 1 under .planning)
     - .planning/sketches/SKETCH-001/.continue-here.md (depth 3)
     - ./.continue-here.md (legacy repo root)
  3. Runs the snippet under bash and asserts each planted file appears
     in stdout.

Same contract; now validated through runtime behavior instead of
regex-on-file.

* fix(#3689): use \\r?\\n in bash-fence regex for Windows CRLF parity

The Windows test-parity guard at tests/windows-test-parity-guard.test.cjs:106
flags any new fence-extraction regex that uses a literal \\n after the
language tag — on Windows CRLF the byte after `bash` is \\r, the regex
silently fails to match, and the extracted snippet is empty. Switch to
the canonical /```(?:bash|sh)\\r?\\n([\\s\\S]*?)```/ pattern.

* fix(#3689): harden extractCheckBlock against missing </step> and CRLF closing fence

Per CodeRabbit review: assert that the closing </step> tag is present before
slicing (otherwise slice(stepStart, -1) silently grabs the wrong block and
the test fails misleadingly), and add \r?\n to the closing fence as well so
Windows CRLF doesn't sneak a stray carriage return into the captured snippet.
