* test(#3148): bound the long tail and delete the allowlist

Migrates the final 170 unbounded sync spawn sites across 49 files, then
removes the allowlist entirely. local/no-unbounded-spawn now runs with no
exemption surface across tests/**: there is no file to add a name to.

drift-detection's throw-native git() helper routes to gitOrThrow -- bare
runGit would have taken 16 call sites quiet on failure. commands.test.cjs
has two independently-scoped runGsdTools/runCli helpers, one already bounded
and one not; they are kept distinct rather than unified, the same trap as the
two same-named git() helpers in Wave 1.

runNpm's bound was erasable. Its options spread callerOptions after the
defaults, so an explicit timeout:undefined silently dropped the 180000ms
bound -- the rule flagged it and was right; it was not a false positive. Fixed
by destructuring with a default, with a test that fails when the default is
removed.

Two sites stay on a raw spawn with an explicit timeout because the seam
cannot express them: one needs shell:true for npm.cmd on Windows, one
redirects stdout to a real fd. Both are the rule's own documented second
option, not an escape from it.

Closure verified rather than asserted: the derivation scan reports 0 unbounded
spawn helpers and 0 unbounded direct git call sites, and a temporary file
carrying an unbounded spawn still errors with the allowlist gone.

Closes #3064.

* test(#3148): close a hole in the guard's own eslint-disable ban

The ban listed only the top level of tests/, so it was blind to 37 .cjs
files under tests/helpers, qa, observability, fixtures and dispatch. With the
allowlist deleted this test is the sole remaining way to detect someone
silencing the rule inline, so the gap was load-bearing: a nested file could
carry an unbounded spawn plus an eslint-disable and pass everything.

Proven before and after. A probe planted under tests/helpers with both was
invisible to the guard and clean under eslint; after making the listing
recursive the guard fails on it. The scanned set goes from 771 files to 808.

Pre-existing since the guard shipped, but this wave is what promoted it to
sole defense, so it is fixed here rather than filed.

Also converts the last hand-rolled throw check to throwIfFailed and the last
re-derived legacy shape to compose toLegacyResult, which makes the epic's
none-remain claim true rather than nearly true. toLegacyResult itself is not
widened -- eight callers depend on its shape and one consumer does not
justify changing a shared contract.

* fix(#3148): correct seam incoherence at the bound and a slow review-lane error path

Two real failures from the remote runner, both fixed at the cause.

The seam could return outcome TIMED_OUT together with exitCode 0. At the
exact bound spawnSync reports ETIMEDOUT while the child has already exited
with a real status, and toSeamResult classified on the error code while
passing status straight through -- an incoherent pair its own boundary test
was written to catch, and did. A status that is not null is direct evidence
the child exited on its own, so it now decides the outcome before the
error-code branches run. process-seam.cjs was deliberately untouched by every
earlier wave; this is a defect in the module itself, kept surgical, with a
unit test that fails against the old logic.

review-lane with an unknown subcommand fell through to its usage error only
after loading the capability registry and building a per-lane plan, which
spawns one child process per lane -- up to twelve. The error path took
~1288ms instead of ~119ms, and under bench load it outran a caller's spawn
timeout and was killed before writing anything, which is the empty stdout and
stderr CI saw. It now fails fast before any of that work begins.

This is the epic's first production change. It is user-facing, so it carries
a changeset rather than a no-changelog label.

* test(#3148): replace a real-race timeout test with a deterministic one

E9 raced git rev-parse against a 1ms bound and assumed git always lost. On a
warm container git finishes first, spawnSync returns status 0 with no error
at all, the seam correctly classifies EXITED, and gitOrThrow correctly does
not throw -- so the test failed on both lanes. A probe confirms a genuine
timeout always carries status null, so this was never the seam misbehaving.

Raising the bound would only lengthen the odds, which is the same defect with
better luck. The test now drives gitOrThrow against a stubbed runGit that
returns a synthetic TIMED_OUT result, so it asserts exactly what it always
meant to -- that a timeout propagates as a throw -- with no timing
dependence. Five consecutive runs are identical where the old one varied.

I wrote this test in Wave 0; it is a real-race test by construction and
CLAUDE.md says to replace those rather than re-run them.

Migrates 69 unbounded sync spawn sites across 24 files. Allowlist 73 to 49.

Two shared helpers move: tests/helpers/graphify.cjs (6 importing suites) and
tests/fixtures/index.cjs, whose three quoted-argument shell strings became
single argv elements rather than whitespace splits.

changeset-lint's throw-native git() helper routes to gitOrThrow; migrating it
to bare runGit would have silently swallowed a failure that is loud today.
ingest-docs goes the other way -- its catch never rethrew, it degraded failure
into data every call site asserts on, so throwIfFailed would have thrown where
the original returned. The design doc said otherwise and was corrected.

tsconfig-noemit runs a real tsc --noEmit and takes a bespoke 180000ms per the
ensure-runtime-build precedent, not the 30000ms build-hooks norm -- that norm
is for a file copy, and sizing against a label rather than the work is the
same error in the opposite direction.

* test(#3147): add toLegacyResult and settle review findings

The seam exposed a throwing adapter (throwIfFailed) but no non-throwing one,
so eight files independently re-derived the same unwrap back to the legacy
{status, stdout, stderr} shape. That is the third time this epic produced N
copies of one mechanism -- seven throw wrappers in Wave 1, fifty-two timeout
constants in Wave 2, eight result adapters here. The pattern is that whenever
the seam does not expose a mechanism, every suite re-derives it.

toLegacyResult now sits beside throwIfFailed, with its own tests.

Two sites are deliberately NOT converted: changeset-cli's runRender and
runRenderIn return {status, report, stderr} from parsed JSON and never a raw
stdout, so they are a different shape family. lint-legacy-dir-name keeps its
local GUARD_TIMEOUT_MS: 30000 matches the build norm numerically but bounds a
lint probe, not hooks bundling, and importing it would encode a coincidence
as a relationship.

Migrates 156 unbounded sync spawn sites across 47 files. Allowlist 120 to 73.

Timeouts are sized from evidence already in the tree rather than a house
default, because this wave spawns installers rather than git plumbing and an
undersized bound does not catch a hang -- it manufactures CI flake, which is
worse, since a flake gets re-run instead of investigated. install.test.cjs
records a real spawnSync ETIMEDOUT at a 60000ms cap on a loaded bench while
another lane passed the same commit in 12.7s, so full installs are bound at
120000ms against that recorded incident.

Also adds an auditable escape to the guard's timeout ceiling. The 600000ms
cap was set in #3143 from partial evidence, but fragment-single-edit-
propagation carries a documented, load-tested 900000ms bound on a run that
chains a full build plus eight generators -- the guard would have rejected a
correct timeout the moment that file left the allowlist. A value above the
ceiling is now permitted only with an inline allow-spawn-timeout-ceiling
marker carrying a non-empty reason. It raises the ceiling; it never waives
the requirement for a bound, which is asserted directly.

install-shared.cjs keeps its hand-rolled assert rather than routing through
throwIfFailed: its message embeds both streams, and throwIfFailed carries
only a trimmed stderr. The message now also names the outcome, so a bounded
timeout reads as such across its 38 importers instead of as
expected null to equal 0.

* test(#3145): extract class-norm timeouts and correct the build-hooks sizing

A pre-PR review found 52 copies of four class-norm timeout constants across
this wave. These are not per-suite fixture bindings -- they are shared facts
about how long a class of subprocess takes, derived from a recorded bench
incident. That norm already moved once (60000 to 120000 after a real
ETIMEDOUT), and 52 copies would have drifted the next time it moved.

Extracts tests/helpers/timeouts.cjs, where each norm is justified once, and
converts the copies. A site that genuinely differs -- a real tsc compile, or
regen:derived -- keeps its own local constant with its own justification.

Also corrects a misclassification: scripts/build-hooks.js was sized as a
build at 120000 in twelve places and 60000 in another, but it compiles and
bundles nothing. Its own header says no bundling needed; it copies pre-built
files and syntax-checks them with vm. Three different values bounded one
script; now there is one.

* test(#3145): fix red CI — lint self-match and a Windows chunk overrun

Two failures on PR 3176.

lint-allow-test-rule-refs read a RuleTester fixture as a real exemption. The
fixture exists to prove an unrelated marker does NOT suppress the rule, so it
carries that marker's literal text as test data. Split via concatenation, the
same idiom no-unbounded-spawn-allowlist.test.cjs already uses for its own
self-match problem. The explanatory comment needed the same treatment.

The Windows shard 3/3 chunk was killed at its 600000ms budget. Output stopped
seven minutes before the kill, so this was an overrun rather than a slow
chunk: regenDerivedPropagatesSingleFragmentEditWithNoSecondSourceSurface runs
regen:derived bounded at 900000ms, which is larger than the whole chunk
budget, so the chunk killer always fires first and it can never complete
there. Both the test and that bound predate this change; modifying the file
pulled it into the Windows targeted set and exposed it. Skipped on Windows
with the reason recorded; the Linux lanes cover it. The 900000 bound and its
ceiling marker are unchanged -- they are correct.

* test(#3145): refresh the stale test-timings cost table

The Windows shard was killed at its 600000ms per-chunk budget. run-tests.cjs
packs chunks by measured duration from tests/test-timings.json, and an
unknown file falls back to the table's median weight -- advisory by design,
but it silently underweights exactly the files that matter.

Four of the failing chunk's 22 files were absent from the table, including
the two heaviest: fragment-single-edit-propagation.install.test.cjs at 230s
(it runs regen:derived) and agent-fragments-emission.install.test.cjs at 79s.
Both were weighted as average, so the chunk's total weight read 53.68 against
a budget of 60 and the packer produced a single chunk.

Regenerated from a passing full-suite run, per the remedy the script itself
documents. 700 to 770 entries, 70 added, 0 dropped -- verified, since
gen-test-timings.cjs replaces the table wholesale rather than merging.

Proven against the real packer: the same 22 files now weigh 103.91 and split
into two chunks. No logic, budget, or timeout was changed; raising a budget
to make a red gate pass is not a fix.

Migrates 180 unbounded sync spawn sites across 19 files. Every previously
unbounded call now carries an explicit timeout with a comment giving the
number and why.

The migration is not a callee swap. execSync and execFileSync throw on a
non-zero exit and the seam never does, so each site was classified first:
sites that rely on the throw route to gitOrThrow, and sites that already read
.status to detect an EXPECTED non-zero -- an intended cherry-pick conflict, a
rev-parse outside a repo driving a skip -- route to the never-throwing runGit
instead, which would otherwise throw on exactly the exit being probed for.

Two same-named git() helpers in worktree-cleanup.test.cjs have different
return contracts, one trimmed and one raw; both are preserved rather than
unified.

Collapses five hand-rolled throw wrappers onto one throwIfFailed in
git-fixture.cjs, which gitOrThrow now also uses so the shape cannot drift.

Allowlist drops 139 to 120; BASELINE lowered to match.

* test(#3144): fix pre-PR review findings

Documents throwIfFailed in the CONTEXT.md glossary and CONTRIBUTING.md --
it became the shared throw mechanism without either doc naming it.

Routes the sixth and seventh hand-rolled copies of the throw shape through
throwIfFailed (worktree-baseref-install, worktree-safety-reap); the first
consolidation missed both.

Converts ci-rebase-check's 8 fixture-setup calls from unchecked runGit to
gitOrThrow so a failed setup step aborts where it fails rather than
surfacing later as a confusing failure against the wrong subject.

Adds 12 direct unit tests for throwIfFailed, which until now was only
exercised transitively.

Splits verify.test.cjs's non-git grep/sed bound off GIT_TIMEOUT_MS.

Adds the ESLint rule local/no-unbounded-spawn, wired into the tests/**/*.cjs
block, plus an allowlist that only ratchets down: a listed file with zero
violations reports its own entry as stale.

The rule resolves renamed destructures and chained requires rather than
matching literal callee names -- both forms exist in the suite today and a
name-only matcher leaves them permanently invisible. It resolves an options
object held in a single-write const, which is what keeps process-seam.cjs,
the bounded reference implementation, from flagging itself.

timeout: 0 and anything above the 600000ms ceiling are rejected as only
nominally bounded.

Adds tests/helpers/git-fixture.cjs so a migrated execSync call site keeps
its throw-on-non-zero contract; process-seam.cjs is unchanged.

* test(#3143): prove the allowlist guards can actually fail

Extracts the D4/D6/D7/D8 checks into pure helpers and drives each against a
synthetic fixture carrying an injected violation. Without this the suite only
proved that today's clean data passes, which a deleted check would also
satisfy.

* fix(#3143): close two ceiling and alias escapes found in review

Nested arithmetic bypassed the ceiling entirely: the numeric evaluator only
resolved a flat literal, so `timeout: 60 * 60 * 1000` (3600000ms, six times
the ceiling) fell through to trusted and reported nothing. The evaluator now
recurses through arithmetic and unary signs with a depth cap.

Alias resolution was traversal-order dependent, not scope dependent: a call
textually above its own require destructure saw an empty alias map and
reported clean. The map is now built in a Program pre-pass.

Also: an explicit timeoutMs:undefined no longer overwrites the git fixture
default via spread, adds the missing seam-routed rule test, and de-duplicates
the repeated try/catch in the fixture tests.

5fd5c81042 test(#3055): add the process seam so a subprocess timeout is expressible as data (#3066)
* test(#3055): add the process seam and route runGsdTools through it

Adds tests/helpers/process-seam.cjs — runNode/runGit/runHook over spawnSync,
each returning a typed discriminated union
{ outcome, exitCode, stdout, stderr, timedOut, signal, killed, code }.
Every call is timeout-bounded; there is no unbounded path.

runGsdTools becomes an adapter over the seam. Its legacy
{ success, output, error, exitCode } shape and retry-once-on-kill behaviour
are preserved byte-identically, so none of its 136 caller files change.

Outcome discrimination was corrected against probed runtime behaviour rather
than assumption: a timeout and a maxBuffer overflow are identical on both
status (null) and signal (SIGTERM), and differ only by code (ETIMEDOUT vs
ENOBUFS). Overflow is therefore classified before timeout. This fixes a live
defect — the previous isKilled() treated an overflow as a kill, retried it for
a second full 60s run, and then reported "host OOM or scheduler contention"
for a child that had merely printed too much.

Also widens the ESLint tests glob from tests/**/*.test.cjs to tests/**/*.cjs,
which brought 31 previously unlinted shared helpers under the same rules their
sibling test files already obey, and fixes the 5 violations that surfaced —
including a bare npm invocation without shell:true in
tests/helpers/emitted-runtime.cjs (DEFECT.WINDOWS-TEST-PORTABILITY), now
routed through the existing portable runNpm helper.

Replaces the spawn body of all 25 local runHook/runGuard/runGate definitions
with a call to tests/helpers/process-seam.cjs. Each wrapper keeps its name,
parameter list, return shape and post-processing (JSON parse, ANSI strip, env
sanitising, field extraction) — only the spawn mechanism changes, so no test
assertion moves.

The 4 bash-driven wrappers use the seam's explicit `interpreter` option rather
than a fourth primitive; it is explicit rather than inferred from the file
extension, because guessing an interpreter from a path fails silently when a
script's name does not match its shebang.

Seven wrappers were previously unbounded and now carry an explicit timeout
sized to what each actually runs, not the seam default. Two of those seven
(gsd-write-guard, lint-docs-command-form) were absent from the issue's
inventory entirely and were found by scanning after the migration.

Adds the CONTEXT.md `### Process seam` glossary entry and a CONTRIBUTING.md
reference section covering the three primitives, the discriminated union, and
the two rules the seam enforces.

Scope disclosure recorded in the phase design notes: the issue scoped three
identifier names. A scan for local helpers that spawn AND return the spawn
result finds 113 across 82 names, 71 of them unbounded, plus 122 unbounded
direct git call sites. This change bounds 25 of those. The remaining surface
is the same defect class and is NOT closed by this PR.

Blocker found in this branch's own diff, independently confirmed by an
isolated reviewer.

A child killed by an external signal — a genuine bench OOM kill — makes
spawnSync return { status: null, signal: 'SIGKILL' } with NO .error field.
The seam's "no error implies EXITED" rule therefore classified it as a clean
exit, and runGsdTools returned { success: false, exitCode: 1 } without
retrying. That silently defeated the #969 kill-discrimination for precisely
the case it was built for: the old isKilled() fired on `signal != null`,
retried once, then threw a labelled resource-starvation error. A real OOM
would have been reported as an ordinary assertion failure.

Adds a fifth outcome, KILLED, for "no error but a signal is set", and makes
the adapter retry on TIMED_OUT or KILLED — reproducing the old
`killed || signal != null || code === 'ETIMEDOUT'` condition exactly.
SPAWN_FAILED still does not retry (matching the old behaviour, where signal
was null). BUFFER_OVERFLOW still does not retry, which remains a deliberate
divergence: the old code retried it because signal was SIGTERM, burning a
second 60s run on a child that had merely printed too much.

All five outcomes verified against the live runtime rather than assumed:
SIGKILL -> killed, exit 0/7 -> exited, timeout -> timed_out (ETIMEDOUT),
>1MB stdout -> buffer_overflow (ENOBUFS).

Three findings from the standards axis of the review, all in this branch's
own diff.

The CONTEXT.md glossary entry this branch introduced was already stale on the
branch's own last commit: it enumerated a 4-member OUTCOME while the code had
5, because the KILLED fix did not update it. That is precisely the drift the
"module changes update Domain-terms" gate exists to catch, so the entry now
lists all five and explains KILLED.

api-coverage-gate-e2e compared an outcome against the raw string 'exited'
rather than OUTCOME.EXITED, the only such outlier; the enum is now imported
and used. A sweep for the other four outcome literals found no further
comparison sites.

Three call sites hand the literal bash flag '-c' to the seam's first
parameter, which the JSDoc described as an absolute script path. Rather than
add a fourth primitive, the contract is corrected to match reality: the
parameter is renamed `target` and documented as the first argv element handed
to the interpreter — normally a script path, but for an interpreter invoked
with an inline program it may be that interpreter's own flag. No behaviour
change.

The remote runner failed on both Linux lanes (node 22 and node 24, identical)
while the same tests passed locally on macOS. Two assertions encoded a
platform-specific behaviour as a cross-platform guarantee.

When spawnSync times out, macOS preserves the child's partial stdout/stderr;
Linux discards it and returns empty strings. Verified on node v26.5.1 both
ways. The seam passes through whatever spawnSync hands it and cannot
manufacture output that was discarded, so the production code was correct —
the tests were wrong.

Both tests now assert the guarantee the seam actually makes on every
platform: outcome TIMED_OUT, timedOut true, and stdout/stderr always being
strings rather than undefined or a Buffer. The partial-content assertions are
retained behind an explicit process.platform === 'darwin' guard so the macOS
coverage is not lost, and the first test is renamed to say what it now
guarantees.

This is the failure mode the remote matrix exists to catch: local macOS
verification would have shipped it.

Windows CI caught two defects the Linux matrix could not.

tests/context-predicates-query.test.cjs passes a 32K-char argv value. On
Windows that exceeds the argv limit and spawnSync fails with code
ENAMETOOLONG, signal null, status null. The seam's fallback rule — "otherwise,
status === null implies TIMED_OUT" — swallowed it, so the adapter retried a
spawn that can never succeed and then threw the resource-starvation error. The
old isKilled() returned false for that shape and returned an ordinary failure
result.

TIMED_OUT is now identified positively: code === 'ETIMEDOUT' OR signal is set.
Anything else carrying an error is SPAWN_FAILED, which covers ENAMETOOLONG,
E2BIG, EACCES and ENOENT alike. The signal clause is what keeps a platform
whose timeout errno differs classified correctly, so the greedy catch-all is no
longer needed.

The second defect is a contract regression I introduced and had claimed
otherwise. That same test asserts `typeof r.exitCode === 'number'`, and
toLegacyShape was returning null for BUFFER_OVERFLOW and SPAWN_FAILED, so the
assertion failed on type. The old code returned `err.status ?? 1` on every
non-retried failure path. The adapter now returns 1 again for both, and the
comment claiming "never coerced to exitCode:1, unlike the pre-seam helper" is
retracted: the seam keeps the richer truth (exitCode null plus a distinct
outcome), the legacy adapter keeps the old numeric contract its callers
actually depend on.

Verified on this host: a 4MB argv yields E2BIG -> SPAWN_FAILED; ENOENT ->
SPAWN_FAILED; timeout -> TIMED_OUT; >1MB stdout -> BUFFER_OVERFLOW; SIGKILL ->
KILLED; clean exit -> EXITED.

c043f2946c fix(#2914): per-PR ack fragments instead of one shared mutable file (#2923)
* fix(#2914): never persist a spent emitted-drift ack on next

tests/emitted-drift-ack.json held 34 spent #2834 entries merged via #2900.
Every entry is scoped to the diff that introduced it (#2789), so once merged
to next it is at the base by definition -- spent and inert. Its presence is
still load-bearing though: each PR rewrites the paths map wholesale, making a
persistent base copy a shared cell. Five of six conflicting PRs in the open
queue collided on this file and nothing else.

Deletes the stale document and adds a push-to-next guard asserting it stays
absent. The guard is deliberately NOT wired into lint:ci -- a PR-lane check
against the base is the #2768 shape #2789 exists to end.

The emitted-drift acknowledgment lived in a single tests/emitted-drift-ack.json
whose paths map every PR rewrote wholesale. That is a shared mutable cell: any
two PRs needing an ack edit the same lines and conflict. Five of six conflicting
PRs in the open queue collided on this file and nothing else.

Acks now live as per-PR fragments under tests/emitted-drift-acks/, the same
shape .changeset/ already uses to solve this exact problem. Two PRs pick
different filenames, so they cannot collide, and fragments lingering on next
are harmless rather than toxic.

The legacy file's 35 entries are MIGRATED into a fragment, not deleted. An
earlier delete-only attempt failed verification twice: the ratchet lost the
spec-phase.md acknowledgment from #2779 and reported a 10-byte growth with no
ack. Relocating preserves every acknowledgment.

The legacy single file is still READ (unioned with the fragments) because five
open PRs carry it; dropping support would break all of them. A duplicate path
key across sources is a hard error, never last-wins.

The push-to-next guard is retargeted accordingly: it now asserts only that the
legacy SHARED file never reappears on next. Fragments may persist harmlessly.

e276cc7f00 enhance(#2778): make the size-ratchet failure name its own remedy (#2780)
* fix(#2778): exempt intentionally-absent paths from the glossary gate

check-glossary-refs asserts that every backticked tests/ token in
CONTEXT.md resolves on disk. tests/emitted-drift-ack.json (ADR-2719
section 3) is absent on a healthy next BY DESIGN — it appears only
inside a PR that needs it, which is what makes touching it the alarm.

It passed before only by accident of backtick pairing: CONTEXT.md's
RULESET entries are themselves backtick-wrapped and contain backticks,
so the token happened to fall outside a code span. Any edit that
shifted the parity exposed it. A gate that passes by luck is not
passing.

The exemption is exact, not a prefix hole: a sibling missing tests/
path still fails, and a test locks that.

* feat(#2778): make the size-ratchet failure name its own remedy

The growth branch stated a requirement and withheld the means of
satisfying it: no ack file named, no schema, no key format, and no
do-not-regenerate line — so the likeliest guess was to hunt for a
baseline that #2724 deleted. Observed live on #2543.

All remediation now comes from one frozen REMEDIATION export whose
example document is rendered from ACK_VERSION, so the taught schema
cannot drift from the schema parseAck accepts. A round-trip test feeds
the printed document back through parseAck.

The report is now built as a typed IR (buildReport) that formatReport
renders, so tests assert on structure rather than prose, per
CONTRIBUTING.md's raw-text-matching rule.

Two defects found and fixed inline while building:
- diffEmitted's validation early-return omitted newFileCapExceeded
  while formatReport reads its length, so the branch that reports a
  failed git diff threw a TypeError instead of naming the problem.
- Printing one complete ack document per failing branch made each read
  as the whole file, so pasting the second over the first silently lost
  an acknowledgment. One document now covers the whole report.

Closes #2778

Found by the 2026-07-24 ADR corpus audit; each mechanism re-reproduced live
against next @ 3eb1cede before filing.

1. docs/adr/1239-gsd-embeddable-orchestration-engine.md linked the
   host-integration capability matrix as `reference/...` from inside
   docs/adr/, which resolves to the nonexistent docs/adr/reference/.
   Three occurrences (the #2584 amendment added two after the audit).
   All now `../reference/...`.

2. src/plan-drift-guard.cts cited docs/adr/0022-source-grounding-drift-guard.md,
   a path that has never existed (git log --all --diff-filter=A returns
   nothing). Corrected to docs/adr/22-plan-drift-guard.md. Comments survive
   tsc and ADR-457 builds at publish, so the bad citation shipped to users --
   verified by grepping the compiled gsd-core/bin/lib/plan-drift-guard.cjs.

3. CONTRIBUTING.md and docs/contributor-standards.md illustrated the ADR
   naming convention with issue #3485 -- a pre-rename number from the
   predecessor repo (get-shit-done-redux) that does not resolve in
   open-gsd/gsd-core. It is dangling, not invented: ADR filenames 3524 and
   3660 show pre-rename numbering reached the 3000s. The worked example now
   uses #2264, which resolves; the one genuinely historical mention is
   annotated rather than rewritten.

4. docs/adr/857-capability-system.md's H1 carried a stale [Proposed] bracket
   contradicting its "Accepted -- ratified 2026-07-17" Status field.
   gen-adr-index.cjs:213 strips the bracket rather than comparing it, so the
   contradiction was invisible to the gate; it is also the only in-repo
   consumer, so removal leaves the rendered index byte-identical.

5. gen-adr-index.cjs's back-link comment still described ADR-857 as Proposed
   and its claim over ADR-0011/ADR-58 as a supersession. Both were restated
   at ratification (the claim became Subsumes; the reciprocals were added).

Regression coverage folds into tests/adr-index-gate.test.cjs rather than a
new bug-* file (lint-regression-test-names): four cases covering link
resolution, H1-bracket-vs-Status agreement, the plan-drift-guard citation,
and the naming worked example. All four fail at the pre-fix tree.

No behavior change. lint:ci exit 0; adr-index-gate 35/35; index regenerates
unchanged (67 ADRs).

* chore(#2691): add the required pr: field to the changeset fragment

docs-lint rejected the fragment with fail_malformed_fragment / missing_pr: the
frontmatter needs both `type:` and `pr:`. The fragment was hand-authored before
the PR existed, so it carried only `type:`.

Note for future work: neither lint:docs nor lint:changeset is part of the
lint:ci chain, so a green lint:ci does not cover these two CI checks. Both were
run directly before this push:

* fix(#2691): drop the false branch clause and repair two more matrix links

Review round 2 on #2692, both blocking findings.

F1: the worked example at CONTRIBUTING.md:101 asserted
'on branch docs/2264-golden-parity-redesign'. That branch never existed --
the ADR file carries the epic number (#2264) while the branch and commit
carry the Phase-0 sub-issue number (#2265, PR #2270, branch
docs/2265-golden-parity-adr). #2264 is therefore the one ADR in the corpus
where filename and branch numbers deliberately disagree, making it the worst
available illustration of 'the issue number becomes your prefix and your
branch'. The branch clause is dropped; the surviving claim is verified
(#2264 is open-and-approved with approved-enhancement, and the file is
2264-golden-parity-redesign.md).

F3: docs/how-to/install-on-your-runtime.md:460 and :476 linked the
host-integration capability matrix as a bare filename from inside
docs/how-to/, which resolves to the nonexistent
docs/how-to/host-integration-capability-matrix.md. Same defect class as
repair #1 in this PR. Both now use ../reference/..., matching the form
already used by the sibling add-or-update-a-host-integration.md:167.

* fix(#2691): take the review minors -- anchors, bracket, ADR path, token order

Review round 2 on #2692, non-blocking findings.

F4: ADR-1239:198,206 read '[capability matrix §codex](...matrix.md)' -- the
link text promised a section, the target carried no fragment. '## codex'
exists at docs/reference/host-integration-capability-matrix.md:85, and the
repo already uses that form (#zcode, #pi in the how-to).

F5: docs/adr/857-capability-system.md:1 had its stale [Proposed] bracket
dropped rather than corrected, making 857 the only ratified ADR with no H1
bracket while four other Accepted ADRs carry one. Restored as [Accepted],
which satisfies the H1-vs-Status invariant and preserves consistency. No
recorded rule mandates the bracket, so this is style, not contract.

F7: docs/CONFIGURATION.md:808 cited adr/1244-runtime-capability-registry-
overlay.md; the actual file is adr/1244-capability-ecosystem.md. Same defect
class as repair #2, one directory over.

F8: test 33 resolved the Status token with STATUS_TOKENS.find(), which
matches by array order rather than by position in the line. Since
STATUS_TOKENS[0] === 'Accepted', a future '- **Status:** Superseded by ADR-X
(was Accepted ...)' paired with an H1 [Superseded] would have reported a
false mismatch. Now resolved by earliest index in the line. No ADR has that
shape today, so this is latent; ADR-857's two-token Status line resolves to
'Accepted' under both the old and new rule.

Changeset updated: five repairs -> seven, adding the two how-to matrix links
and the CONFIGURATION.md ADR path, plus the (#2691) issue backlink that 46
of the other 47 fragments carry.

Not addressed here: F2 (widening the guard to resolve anchors and walk docs/
recursively) is filed separately as #2704, approved-enhancement.

1c1af70a4b refactor(#2724): delete the committed golden fixtures and size baselines (#2767)
* test(#2724): delete golden-install-parity fixtures, test, and generator

Removes the 19 committed path->hash manifests, the two per-file size
baselines, tests/golden-install-parity.test.cjs, and
scripts/gen-golden-install-parity-zcode.cjs. These were pure functions
of the source tree (ADR-2719); the differential attribution check
(tests/emitted-attribution.test.cjs + tests/emitted-provenance.test.cjs)
is now the sole gate for emitted-artifact propagation.

tests/fixtures/install-tree/*.json and tests/golden-install-tree.test.cjs
are unchanged (ADR-2719 section 7 exception).

Follow-up commits fix the resulting bookkeeping: scripts/ci-test-scope.cjs's
existence guard, .gitattributes, package.json scripts, the emitted-provenance
totality guard's IO, the differential check's baseline acquisition, CI
wiring to publish/restore the baseline artifact, and docs.

* refactor(#2724): make the differential attribution check self-sufficient

Three fixes required to delete the golden fixtures without breaking CI:

- scripts/ci-test-scope.cjs: remove tests/golden-install-parity.test.cjs
  from the three rules that named it. #2759's missingRuleTestFiles guard
  hard-throws at module load if a rule names a test file absent from
  disk, which would break the changes job on every PR the moment the
  fixture-deletion commit landed.

- tests/helpers/emitted-provenance.cjs: loadManifests() read the
  committed golden fixture directory. With that directory deleted at
  every future ref, this would throw at module load forever, taking
  the Phase 2 totality guard down with it. Rebuilt from real installer
  spawns (MANIFEST_FAMILIES + runMinimalInstall + buildParityManifest),
  the same shape emitted-runtime.cjs's currentManifests() already uses.

- tests/emitted-attribution.test.cjs / tests/helpers/emitted-runtime.cjs:
  the real-tree test's baseline acquisition swaps from
  baselineManifestsAtRef(base) (git show at a ref that no longer carries
  fixtures) to resolveBaseline()'s documented precedence: env, then the
  on-disk cache, then an in-job build. The build fallback
  (buildBaselineAtRef, new) checks out base into a throwaway git
  worktree and runs the new scripts/gen-emitted-baseline.cjs there --
  no npm ci needed, since bin/install.js and the test helper shells are
  Node-builtins-only. That script also publishes the baseline artifact
  from CI's push-to-next job (wired in a follow-up commit).

* refactor(#2724): retire the merge-driver bridge and per-file size baselines

The Phase 1 bridge (#2721) is retired now that the artifacts it guarded
are deleted: scripts/git-merge-regen-driver.cjs, its test, and the
'setup:merge-driver' npm script are removed, and the .gitattributes
merge=gsd-regen/linguist-generated block for the three deleted-path
globs is dropped. tests/fixtures/install-tree/*.json keeps its normal
merge behavior, unchanged (ADR-2719 section 7).

scripts/update-size-baseline.cjs and its test are removed: their sole
purpose was regenerating tests/workflow-size-baseline.json and
tests/agent-size-baseline.json, both deleted. The 'size:baseline' npm
script and its step in 'regen:derived' go with it. The per-file
baseline describe blocks in tests/workflow-size-budget.test.cjs and
tests/agent-size-budget.test.cjs are removed for the same reason; the
independent loose-tier hard caps are untouched. The differential
attribution check's size ratchet (tests/emitted-diff.cjs, already
shipped in #2723) is the replacement anti-creep mechanism.

'npm run gen:golden' is replaced by 'npm run gen:install-tree', which
keeps regenerating tests/fixtures/install-tree/*.json (the one artifact
family ADR-2719 section 7 keeps committed); tests/golden-install-tree.test.cjs's
error messages point at the new command name.

tests/golden-parity-single-source.test.cjs's anti-divergence guard
(#2266) is retargeted from the two deleted golden-parity consumers to
their two replacements (tests/helpers/emitted-runtime.cjs and
tests/helpers/emitted-provenance.cjs), which import buildParityManifest
the same way — the divergence risk the guard exists for is unchanged.

Also wires CI: a new publish-emitted-baseline job runs
scripts/gen-emitted-baseline.cjs after a push to next and caches the
result keyed on the sha; the test and test-full jobs restore that cache
on pull_request events, keyed on the PR's base sha, and export
GSD_EMITTED_BASELINE for tests/emitted-attribution.test.cjs's real-tree
test to pick up.

* docs(#2724): flip ADR-2719 to Accepted and update contributor docs

Status: Proposed -> Accepted. Regenerated docs/adr/README.md index.

CONTRIBUTING.md, docs/TESTING-SUITES.md, and CONTEXT.md (RULESET.
EMITTED_ATTRIBUTION, RULESET.WORKFLOW_SIZE_BUDGET, RULESET.
AGENT_SIZE_BUDGET, and the Emitted Artifact Provenance glossary entry)
no longer point at the deleted golden-install-parity fixtures, size
baselines, gen:golden, UPDATE_GOLDEN, or the setup:merge-driver /
git-merge-regen-driver.cjs bridge. Editing shipped content now
requires zero manual fixture regeneration, documented against the
differential attribution check instead of the deleted commands.

* docs(#2724): add changeset for removed golden-parity commands

* fix(#2724): drop stale scripts/update-size-baseline.cjs glossary ref

check-glossary-refs.cjs verifies every backtick-wrapped scripts/*.cjs
token in CONTEXT.md resolves to a real file. The RULESET.
EMITTED_ATTRIBUTION rewrite named the deleted script inside backticks,
which the checker reads as a live reference, not historical prose.

* test(#2724): retarget ci-test-scope tests off the deleted golden test

tests/ci-test-scope.test.cjs asserted specific RULES entries select
tests/golden-install-parity.test.cjs, and that every rule selecting it
also selects both emitted gates. Both premises broke when the golden
test was deleted (#2724): the deleted filename never re-appears in
targeted_tests, and there was no longer a third file for the gates to
travel alongside. Retargeted the two selection describe blocks to
assert tests/emitted-provenance.test.cjs directly (the drift guard the
golden gate's rules were retargeted to), and simplified the third block
to assert the two emitted gates always travel together, without
reference to the golden filename.

* docs(#2724): repoint two contributor how-to guides at the differential check

Both guides told contributors to regenerate a baseline against
tests/golden-install-parity.test.cjs, which #2724 deletes. Repointed
at the differential attribution check (tests/emitted-attribution.test.cjs,
ADR-2719), which needs no manual regeneration step.

* fix(#2724): repair phase6-capstone-conformance's deleted-baseline read

An independent orthogonal review caught a real regression this branch
introduced into a test file the branch's diff never touched:
tests/phase6-capstone-conformance.test.cjs read
tests/workflow-size-baseline.json (deleted earlier in this branch) with
no fallback, so the whole suite would throw ENOENT the moment this
branch landed. The test's actual intent — prove the host-loop workflow
files are real, tracked, non-empty docs — is preserved by asserting the
live byte count via the same shared counter (scripts/workflow-size.cjs)
the size guards already use, instead of a committed snapshot.

Also, from the same review: a stale doc comment in
scripts/workflow-size.cjs still named the deleted
scripts/update-size-baseline.cjs as a consumer, and
buildBaselineAtRef's cleanup in tests/helpers/emitted-runtime.cjs left
two fs.rmSync calls unguarded against masking the primary result/error,
inconsistent with the try/catch already wrapping the git cleanup beside
them. Both fixed. A doc comment was added to baselineFamilyNamesAtRef
explaining why it (and its siblings) are kept despite having no
production caller post-cutover — they still answer real questions
about refs that predate the cutover.

* fix(#2724): repair three real regressions found by remote verification

1. tests/emitted-provenance.test.cjs's two hostile-input tests
   (non-object manifest, unreadable fixture) drove loadManifests(tmp)
   and monkeypatched fs.readFileSync, both premised on the deleted
   fixture-directory read this branch already replaced with real
   installer spawns -- the negative assertions silently stopped firing.
   loadManifests() now accepts injected {families, install, build,
   clean} (defaulting to production values), giving the tests a real
   seam to drive a bad build result and a build failure through the
   ACTUAL loader instead of a reimplementation, and added coverage that
   clean() still runs on both paths.

2. .github/workflows/test.yml's two 'Export GSD_EMITTED_BASELINE'
   steps hardcoded shell: bash, which is wrong on windows-latest (native
   pwsh) and on test-full's macos-latest legs (native zsh per that job's
   own matrix) -- the repo's H1 shell policy (tests/policy-shell-pinning
   .test.cjs) caught it. Replaced the inline bash script with
   scripts/ci-export-emitted-baseline-env.cjs, a plain Node script: a
   bare 'node <path>' command line has no shell-specific syntax, so it
   runs correctly under bash, zsh, and pwsh without a shell override.

tests/phase6-capstone-conformance.test.cjs's deleted-baseline read
(caught by the same remote run, at a commit prior to this one) was
already fixed in d0c3b1242 and is not touched here; verified still
passing after these changes.

* fix(#2724): revive ADR-1610's new-file size cap inside the differential

An isolated review caught a real regression: deleting
tests/workflow-size-baseline.json silently dropped NEW_FILE_CAP
(ADR-1610 Decision point 3, the Codex project_doc_max_bytes anchor)
with no successor. tests/helpers/emitted-diff.cjs's size ratchet
already 'continue's past any file absent from sizeBaseline -- exactly
the files this cap exists to bound -- so a brand-new workflow file
sized 32,769-40,960 bytes passed CI clean and shipped, then risked
silent truncation at the Codex anchor at runtime. ADR-1610 is Accepted
and never referenced anywhere in this branch.

Fix: NEW_FILE_CAP=32768 revived inside emitted-diff.cjs's own
size-ratchet loop, keyed off the SAME hasOwnProperty(sizeBaseline,
name) signal the growth check already computes -- 'new' is exactly
'present in sizeCurrent, absent from sizeBaseline'. Not ack-able,
matching the tier hard caps it sits beside: the fix is extraction, not
an acknowledgment entry. Documented, disclosed narrowing: the pure
differential module cannot see XL_WORKFLOWS/LARGE_WORKFLOWS tiering
(tests/workflow-size-budget.test.cjs's classification), so a
legitimately large new file must extract rather than tier in, one
release earlier than an existing file would need to. ADR-1610 itself is
left unamended -- this restores its decision rather than re-litigating
it.

Also fixes a stale comment plus a redundant real 19-installer-spawn
assertion left over from the pre-injection-seam version of
tests/emitted-provenance.test.cjs's build-failure test, and annotates
3 of 4 stale golden-fixture citations in
docs/reference/host-integration-capability-matrix.md as superseded
(the 4th is an accurate historical PR narrative, left alone).

* fix(#2724): repair three red CI defects on the golden-fixture cutover

Windows-only provenance false attribution (defect A): the `hooks-built`
provenance rule attributed `hooks/<name>.cmd` to itself. Those shims are
Windows-only installer output (ensureCodexHooksJsonSessionStart /
ensureCodexHooksJsonEvent, both in src/runtime-hooks-surface.cts) wrapping
the same-named `.js` hook — no `.cmd` file is ever tracked in the repo, so
the self-attribution resolved to a path that exists on no platform. Only
windows-latest ever emits the key, so this only failed there. Fixed by
special-casing `.cmd` inside the SAME `hooks-built` rule (not a dedicated
rule) — a dedicated rule would match zero paths, and therefore report as a
dead rule, on every non-Windows lane of the same totality guard. `sources`
already supported per-match functions; `transforms` is extended to support
the same shape so the attribution can vary by match within one rule.

Baseline bootstrap was structurally impossible (defect B): `buildBaselineAtRef`
ran `scripts/gen-emitted-baseline.cjs` from INSIDE the base-ref worktree, but
that script is new in this PR and therefore absent at any base ref that
predates it — every call failed closed with "Cannot find module". Fixed by
running the PR checkout's own generator against the worktree via a new `--dir`
parameter, decoupling "which copy of the script runs" from "which tree it
measures" (`currentManifests`/`currentSizes` gained a `repoRoot` override,
threaded down to `runMinimalInstall`'s new `installScript` override). This is
not just a bootstrap fix: a differential needs ONE measurement schema applied
to both sides, or the two stop being comparable the moment that schema
evolves — running each side's own copy would silently reintroduce that risk.
Verified locally end-to-end against real origin/next: resolves a valid
{version, sha, manifests, sizes} artifact with the correct sha and no leaked
worktree.

Changeset placeholder (defect C): `pr: 0` -> `pr: 2767`, which is what let
docs-lint evaluate the fragment for the first time; it already passes
(docs/TESTING-SUITES.md and friends already document the removed scripts).

Also fixed while in this file: an eslint no-unused-vars warning surfaced by
the changed lint run (unused `cleanup` import in
tests/emitted-provenance.test.cjs).

Added regression coverage for both A and B: a cross-platform spot-check that
drives the real hooks-built rule against `.cmd` keys directly (not through a
real Windows install), and a real-tree test that drives buildBaselineAtRef
against a base ref verified (via git cat-file) to lack the generator, both
skipping honestly rather than false-passing when their precondition does not
hold.

Two isolated-review findings on PR #2767:

- `hooks-built`'s `.cmd` branch attributed the Windows shim's bytes to the
  wrapped `hooks/<name>.js` script, asserting a byte-provenance link that
  does not exist — traced against buildCodexHookWindowsShimIR
  (src/runtime-hooks-surface.cts), only the script's NAME (a literal in that
  same file) flows into the .cmd bytes, never its content. Point `sources`
  at HOOKS_WINDOWS_SHIM_SRC instead, matching the code-derived convention
  used elsewhere in the table. Since `sources` is checked before
  `transforms` in the differential, the wrong mapping silently excused any
  .cmd byte movement caused by editing the wrapped .js file.

- The `buildBaselineAtRef` regression test skipped unless a resolvable base
  ref still lacked scripts/gen-emitted-baseline.cjs — true only until this
  PR merges, after which every base ref carries the file and the test skips
  forever with zero ongoing coverage. Rebuilt hermetically: synthesize the
  missing-generator condition in-place via git plumbing (a throwaway commit,
  child of HEAD, with just that one file removed from a scratch index),
  never touching the real working tree, HEAD, or index, and never depending
  on ambient history or remotes.

The runner container mounts the repo at a path owned by a different uid than
the process running the suite, so git's dubious-ownership protection refuses
every git operation there. GitHub Actions never hits this because
actions/checkout registers the workspace as safe automatically; this
runner's container does not.

buildBaselineAtRef is the production build-fallback the sole remaining
emitted gate depends on (resolveBaseline's in-job-build leg), not just a
test helper, so the fix is in the shared git() wrapper (emitted-runtime.cjs)
that every caller — resolveChangedPaths, resolveBase, buildBaselineAtRef's
worktree add/remove/prune, and the hermetic regression test added in the
prior commit — funnels through, plus gen-emitted-baseline.cjs's own
rev-parse (now reusing that same wrapper instead of a second execFileSync,
so the fix has one source of truth). Each call declares -c
safe.directory=<the exact directory it already operates on>, never the *
wildcard.

Audited every other helper on this surface (emitted-diff.cjs,
emitted-baseline.cjs, install-shared.cjs) for the same gap: none of them
shell out to git at all.

Tests precede the implementation per the TDD gate. The driver module does not
exist yet, so tests/git-merge-regen-driver.test.cjs fails at require time; the
contributor-standards parity assertions fail against next as it stands today,
where the standards doc names two CONTEXT.md headings that have never existed.

The golden parity manifests and the two size baselines are pure functions of
the source tree, so their only correct merge is "recompute" -- something git's
ours/theirs interface cannot express. 140 of 143 conflicted-file instances
across the open PR queue are these files.

The driver deliberately does NOT regenerate. Four probes established that at
merge-driver time neither the working tree nor the index reflects the merge:
both hold the ours side, a file added by theirs does not exist yet, and
MERGE_HEAD is unwritten. Git also invokes the driver once per conflicted path
(20 here). A regenerating driver would therefore read the ours-side tree and
emit a plausible-but-wrong hash manifest -- worse than a conflict, because a
conflict is visible. So it accepts %A, runs zero subprocesses, records the
resolved paths, and prints one notice pointing at npm run regen:derived.
Staleness stays caught where it already was, by golden-install-parity in CI.

Every failure path degrades toward today's behaviour (a normal conflict).
install-tree is deliberately excluded per ADR-2719 section 7.

Also folded in, per the no-defer rule: workflow-size.cjs claimed .md files have
no eol=lf in .gitattributes; git check-attr shows eol: lf, set by .gitattributes
line 2 since #1088.

Adds the how-to a contributor actually reaches for when the generated parity
manifests or size baselines conflict, in both places they would look: the
merge-conflict path in CONTRIBUTING.md and the full guide in TESTING-SUITES.md,
including what the driver deliberately does not do (it does not clear GitHub's
CONFLICTING label, and it does not regenerate mid-merge).

Also scopes the new contributor-standards parity assertion to the doc's own
CONTEXT.md section. Its first run flagged `## Decision`, `## Consequences` and
`## Standards followed`, which the doc attributes to an ADR body and a PR body
rather than to CONTEXT.md -- a doc-wide extractor would have demanded CONTEXT.md
grow headings that do not belong to it.

* fix(#2721): stop passing %P to the merge driver — shell injection

The isolated adversarial review found, and I independently reproduced, local
arbitrary command execution.

Git does not invoke a merge driver with an argv array. It substitutes %O %A %B
%L %P textually into the configured string and runs the whole thing through a
shell, and $(...) executes inside POSIX double quotes -- so quoting the
placeholder does not neutralise it. %O/%A/%B are git-generated temp names and
%L is an integer, but %P is the file's own path, chosen freely by any
contributor. A branch renaming a covered fixture to
evil$(touch PWNED_SENTINEL).json executed that command on the machine of every
maintainer who merged it, and the merge still reported success.

Fix removes the input rather than filtering it: %P is no longer registered, so
the driver receives no attacker-controlled argument at all. The marker records
a count instead of path names. A metacharacter filter would have been a guess
about shell grammar; passing nothing is a property. Re-ran the identical
exploit against the fixed command: nothing executed, conflict still resolved.

Two regressions guard it -- a platform-independent assertion that the
registered command carries no %P, and a real merge driven by the actual
planInstall output with a $(...) filename.

Also from review: CLI dispatch had no coverage at all (CONTRIBUTING's
"CLI and command routing" matrix), which is why runInstall/runStatus now take
{repoRoot} -- hardcoding REPO_ROOT was what made them untestable. Renamed
planResolution to resolveAndRecord since the plan* prefix promised purity it
did not have. Reconciled the eleven-vs-twelve generator count across
CONTEXT.md, CONTRIBUTING.md and the changeset.

* test(#2721): scope safe.directory for the check-attr helper

The 66f4d85a run failed 11 assertions, all in the .gitattributes scoping block,
with "fatal: detected dubious ownership in repository at '/work'". The test
container checks the repo out at a path its user does not own, so git refuses
check-attr outright. Everything else passed (27,185).

`check-attr` is a pure read of .gitattributes -- no hooks, no filters -- so the
exemption is scoped to that one invocation. It is deliberately NOT applied to
the driver's own production `git config` calls, which run in the user's own
clone and should keep the protection.

The plex2 run on bdfd0856 left exactly two failures, both this test: it still
asserted the pre-fix command string, i.e. the vulnerable behaviour. Deleted
rather than relaxed, per RULESET.TESTS.delete-bad-tests -- its useful half is
already covered, in both directions, by
registeredDriverCommandNeverPassesThePlaceholderForTheFilePath.

The e2e helper hand-rolled its own driver registration, and still carried %P.
That meant the five real-git tests were not exercising the production command
string at all -- planInstall could drift and they would keep passing. They now

ADR-1411 reasons only about a resolution miss. It is silent on input that
is present but not usable, which is how five engine read paths (#1879) could
fold an unusable input into the value meaning 'genuinely absent' without
contradicting an Accepted ADR.

Read together, ADR-1411 and ADR-227 converge and do not license throwing as
the cluster's answer: ADR-227 requires malformed input to be coerced rather
than propagated and carves out only genuinely-fatal fields, while ADR-1411
already permits a fallback provided it is 'a visible value, not a silent
substitution'. The defect in these five sites is therefore not that they fall
back but that they fall back invisibly.

Records the pattern that follows: every current return value is preserved, and
the cause is made visible in-band where the result already carries a
provenance envelope, or out-of-band via a deduplicated stderr diagnostic where
it returns a bare value it cannot extend. Throwing stays confined to ADR-227's
genuinely-fatal carve-out, decided per call. Also names the per-applier caller
audit and the lint-resolution-provenance registry gap.

Found while amending ADR-1411, which names this dedup guard as the pattern
five downstream PRs (#1880-#1884) will adopt - shipping the ADR without the
fix would have propagated the footgun to each of them. Folded in here per
CLAUDE.md's no-defer rule rather than filed.

RED verified on 3c4895841 (test only, no fix): linux-node22 reported
'FAIL tests/config-loader.test.cjs - the documented per-process
warning-state reset must clear the unknown-key dedup set too'.

TypeScript source of truth compiled into gsd-core/bin/lib/*.cjs, so it is
the most-edited user-facing path in the repo and the omission sends any

Also documents that the lint reads GITHUB_BASE_REF, which only CI sets, so
running it bare locally reports success without evaluating the branch. This
PR hit exactly that: a local run said ok_fragment_present and CI failed
fail_missing_fragment on the same diff.

Found while opening this PR; folded in per the no-defer rule.

Restores: the unconditional-diagnostic clause (ADR-227's GSD_DEBUG opt-in
was never implemented, so citing it as the precedent was wrong), the dedup
key, #1882 folded into the out-of-band mechanism instead of a fourth
mechanism-less category, the narrowed caller-audit rationale, and the
test-methodology clause.

9ade602219 docs(#2585): single lockfile-driven bootstrap path in CONTRIBUTING.md (#2675)
Getting Started opened with `npm install`, contradicting both the `npm ci` quick
start fifteen lines below it in the same file and docs/contributing/bootstrap.md,
which states `npm ci` is required so installs are reproducible, lockfile-driven,
and fail fast when package-lock.json is out of sync. A contributor taking the
shortest path — the first copyable block — got the wrong bootstrap contract.

Rather than correct `npm install` in place and leave two near-identical blocks,
the duplicate "Bootstrap your environment" quick start is folded into Getting
Started so CONTRIBUTING.md carries exactly ONE fresh-checkout path, in the
canonical order from bootstrap.md (nvm use -> npm run check:env -> npm ci), and
names bootstrap.md as the source of truth for everything else (fnm/asdf/mise, the
environment validator, daily commands, troubleshooting). That satisfies the
issue's "keep bootstrap.md as the source of truth rather than introducing another
variant" — three competing variants become one pointer plus one sequence.

No regression test: the change is prose in a contributor guide, not a runtime
contract, so a readFileSync+includes assertion would be exactly the source-grep
test scripts/lint-no-source-grep.cjs rejects.

Adds tests/fixtures/representative/ — a permanent corpus of verbatim,
incident-sourced fixtures (never author-invented) from #2286, #2347,
#2365, #2366, each labeled with its expected gate verdict in a
MANIFEST.json and driven through the real CLI gate entrypoint via
tests/representative-corpus.test.cjs.

Adds a document-shaped fast-check property test alongside the existing
writer-seeded bijection test in tests/api-coverage.test.cjs: the existing
generator produces rows and renders them through the writer, so the
document shape is a constant and it cannot fail against a decoy table;
the new one generates the document space instead.

Two gates (#2365, #2347) are still open, so their corpus/property
assertions are marked with node:test's official `todo` option — the test
executes and reports its failure without affecting the process exit code
(https://nodejs.org/api/test.html#test-options). The audit-uat corpus
(#2286, fixed by #2317) is a normal passing assertion, proving the
methodology works end to end and not just cataloguing gaps.

Records the fixture-provenance rule in CONTRIBUTING.md: a gate's fixtures
may not be derived from the gate's own writer, grammar, or docstring
examples; a negative fixture must come from a source that doesn't know
the gate exists.

No production src/*.cts changes — validation only, per #2371's scope.

gsd-test's own verdict, exactly as if it weren't marked todo — proven
by an actual gsd-test run against this branch, which reported
outcome:"failed" with all six todo-marked assertions (the property
test plus five representative-corpus fixtures) in the failure list,
each carrying the correct raw node:test `todo` field the tool's parser
simply doesn't read.

Replaces todo with characterization: MANIFEST.json now carries both
the correct target verdict (expected*) and the exact current observed
verdict (currentBuggyOutput, directly verified against live CLI
output for all five fixtures). Tests assert currentBuggyOutput — an
honest, non-vacuous pin of today's known-broken reality that passes
today and will fail loudly the moment the referenced fix changes the
observed output, at which point the assertion should be flipped to
expected* and currentBuggyOutput deleted.

The document-shaped property test switches from throwing fc.assert to
non-throwing fc.check (returns RunDetails per fast-check's own docs)
and asserts report.failed === true directly, for the same reason.

Updates all prose (CONTRIBUTING.md, the fixture READMEs) that
previously claimed todo would be respected — that claim was
factually wrong for this repo's actual tooling and must not ship.

The release changelog is title-driven: release.yml generates "What's Changed"
from PR titles, then format-github-release-notes.cjs buckets each line by its
conventional-commit prefix and relies on a `(#<issue>)` in the title to render
the issue link. Both rules were enforced only socially, so titles like
`fix(core): ...` (no issue link) and `[security] fix(...): ...` (leading tag
defeats the `^fix` bucket anchor -> mis-filed under Enhancement) silently broke
the changelog, landing on the maintainer as release-time cleanup.

Extract the title matcher into one shared module consumed by BOTH the changelog
classifier and a new PR-title CI gate, so a title that passes the gate cannot
mis-bucket in the changelog (single source of truth).

- scripts/lib/conventional-title.cjs (new): classifyBucket + evaluatePrTitle +
  the anchored regexes. One matcher, two consumers.
- scripts/release-notes/format-github-release-notes.cjs: classifyTitle now
  delegates to classifyBucket (behavior preserved; existing tests green).
- .github/workflows/pr-title-validator.yml (new): runs evaluatePrTitle on
  pull_request opened/edited/reopened/synchronize, for ALL authors (the drift

The workflow checked out the base branch (next) as a trusted policy source, but
the shared matcher (scripts/lib/conventional-title.cjs) is introduced by this PR
and does not exist on next yet — so require() failed and validate-title errored
on its own introducing PR. Check out the PR's merge ref instead: the matcher
under review is present, the check is self-consistent, and a fork pull_request
runs read-only with no secrets, so running the PR's own pure-string regex is
safe.

* fix(#1549): move conventional-title.cjs out of installed scripts/lib/

bin/install.js bundles every file under scripts/lib/ into the user-installed
payload (the changeset CLI's dependencies), and install.test.cjs (#935) asserts
that exact set. The new matcher is release/CI tooling that must NOT ship to
users, so placing it in scripts/lib/ both broke the install manifest test and
would have shipped dead code. Relocate it next to its consumer in
scripts/release-notes/ (which the installer does not copy) and update the three
require paths (classifier, workflow, test) + the CONTRIBUTING reference.

install.test.cjs now 125/125; conventional-title + release-notes suites green;
lint:ci clean.

* fix(#1549): load title matcher from trusted base ref, not PR code

Addresses review (Solvely-Colin + trek-e): the gate checked out the PR
merge ref and require()'d evaluatePrTitle from PR-controlled code, so any
future PR could edit conventional-title.cjs to return { valid: true } and
wave its own malformed title through — a self-bypassable required check.

Load the matcher from a base-branch checkout instead (ref:
github.event.pull_request.base.ref), the same trusted-policy-source pattern
pr-target-validator.yml already uses. The PR can change its title but not
the ruler that measures it. An existsSync bootstrap guard skips the check
when the matcher isn't on the base branch yet (the introducing PR); every
PR after merge is fully gated. This keeps the single shared matcher (#1549's
whole point) rather than forking the regex into the workflow.

Also per review:
- add tests/conventional-title.property.test.cjs (fast-check): any
  `type(#n): summary` round-trips to valid; evaluatePrTitle/classifyBucket
  are total functions (never throw).
- pin the `fix(#):` zero-digit boundary as missing-issue-ref.

#2551/#3182/#2361 are pre-migration get-shit-done-redux issue numbers with no
equivalent in open-gsd/gsd-core; they mislead triage and manufacture phantom
blockers. Repoint to real successors (#717 byte-budget rework, #720) or rewrite
as prose referencing the discuss-phase/modes progressive-disclosure split.
Correct co-located 'line budget'/'<500 lines' framing to the byte-based reality
(#717). Add a CI guard (tests/no-phantom-issue-refs.test.cjs) that fails if a
phantom ref is reintroduced. SSH-key patterns (id_ed25519) left untouched.
No user-facing runtime behavior change.

e4f0910d62 test(#1074): agent-size-budget per-file baseline + line→byte rebase (PR 3/3) (#1097)
Extends the #1074 scheme to tests/agent-size-budget.test.cjs, which used the
same assertTightCeiling tier ratchet but was still line-based (never rebased in
#717). Completes the migration — the last part of the #1074 epic.

- Rebase agent sizing from lines to LF-normalized bytes (#717/#683).
- Delete the 'SIZE: tier anti-creep' describe (3 assertTightCeiling tests);
  add a per-agent baseline (tests/agent-size-baseline.json) as the primary
  anti-creep, and byte hard caps (XL 56 KiB / LARGE 48 KiB / DEFAULT 24 KiB),
  each above its tier high-water with real headroom. No separate new-file cap:
  a net-new agent is DEFAULT-tier, already bounded by the DEFAULT cap.
- Keep the agent-classification tests verbatim.
- scripts/workflow-size.cjs: add generic measureMdFiles(dir, predicate)
  (workflows + agents share one byte-measurement path); measureWorkflows now
  delegates to it.
- scripts/update-size-baseline.cjs: one 'npm run size:baseline' now regenerates
  BOTH the workflow and agent baselines (gsd-* filter for agents).

Rebased onto next after PR 2/3 (#1096) merged: replicate the
scripts/lib/workflow-size.cjs -> scripts/workflow-size.cjs move (PR 1/3) across
the generator and the agent test's require; regenerate the agent baseline
against current agents (a uniform +170 B preamble drift on all 33 since
authoring).

Addresses the #1097 review (trek-e):
- BLOCKER (acceptance criterion 5): document the agent contract in CONTEXT.md.
  Adds RULESET.AGENT_SIZE_BUDGET (caps 57344/49152/24576, per-agent baseline,
  dual size:baseline, shared measureMdFiles seam) and disambiguates it from the
  separate DEFECT.AGENT-FILE-SIZE-CAP-BREACH 45K-CHAR guard (two units, two
  purposes).
- Docs: now that #1096's docs/TESTING-SUITES.md "Workflow size budget" section
  is in next, fold in the agent coverage here (renamed to "Workflow & agent
  size budget"): agent caps + per-agent baseline + the how-to + reference rows,

b055ca14e2 test(#1074): swap workflow size enforcement to baseline + loose hard caps (PR 2/3) (#1096)
Completes the #1074 migration for workflows. The per-file baseline (PR 1) is
now the primary anti-creep guard, so the tier-max tighten-only ceilings are
retired here.

- Delete the 'SIZE: tier anti-creep' describe (3 assertTightCeiling tests) and
  the GRACE constant — per-file baseline already guards every file by name,
  strictly stronger than max(tier).
- Convert the per-file tier test into 'SIZE: workflow tier hard caps': absolute
  red lines (XL 96 KiB / LARGE 60 KiB / DEFAULT 40 KiB) that mean 'extract, do
  not raise', each with real headroom above its high-water file.
- Add a 32 KiB (Codex project_doc_max_bytes) cap for net-new workflow files not
  yet in the baseline and not explicitly tiered.
- Rewrite the header doc comment for the new two-guard model; drop the
  assertTightCeiling import (now unused here; still exported + used by the agent
  test until PR 3).

Addresses the three PR-2 items from the #1089 review:
- Finish the enumeration consolidation (Minor #1): the tier hard-cap and
  new-file guards now read both their file list and byte sizes from
  measureWorkflows()/listWorkflowStems(), removing the inline readdirSync +
  per-file byteCount split-brain. Enumeration and measurement share one source.
- Rewrite CONTEXT.md RULESET.WORKFLOW_SIZE_BUDGET (Minor #2): stale caps
  (XL<=90000/LARGE<=54000/DEFAULT<=38000) replaced with the baseline-first
  model, current hard caps, the new-file anchor, and the size:baseline
  remediation.
- Ship docs (contract-change requirement): a Diataxis how-to + reference for
  the size guard in docs/TESTING-SUITES.md, beside the sibling regression-name
  ratchet (the 'file grew, CI red -> npm run size:baseline, commit the one-line
  diff, justify or extract lazily' workflow). CONTRIBUTING.md's workflow-tree
  note is updated to the baseline model and points at the new guide.

Negative proof: each guard fails independently when violated (new-file cap at
33 KB; hard cap with baseline current; baseline on any per-file growth).

* fix(#1006): harden render --preview against fragment parse failures

`render --preview` wrote `report.preview` unconditionally. When a `.changeset`
fragment fails to parse, `cmdRender` early-returns with `{exitCode:1, report:
{failures}}` and NO `preview` key, so `process.stdout.write(undefined)` threw
ERR_INVALID_ARG_TYPE and the rc release job's "Preview CHANGELOG" step died with
a cryptic TypeError that masked the real cause.

Guard the preview write on `typeof report.preview === 'string'` (ADR-227: shape,
not just type); when absent, fall through to the existing failure reporter that
names the offending fragment and exits non-zero — identical to a non-preview
render. Also backfills the stray placeholder `pr: 0` -> `pr: 939` in
.changeset/936-convergence-inline-plan-phase.md that triggered the live failure.

Regression test (red-then-green verified) added at the render --preview seam.

placeholder) silently merged to `next` and only detonated later in the rc
release job. This is the upstream prevention for #1006 — the crash hardening
turns the failure into a clear message, this stops the bad fragment ever
reaching the release path.

evaluateLint now accepts `fragmentFailures` and fails with the typed reason
`fail_invalid_fragment` (naming each offending file) before the existence/
opt-out checks — a malformed fragment beats `no-changelog`, since it will break
the render regardless. main() reads + parseFragment()s every changed fragment:
a deleted fragment (not on disk) is skipped, a present-but-unreadable one fails
closed. Tests assert on the typed LINT_REASON enum (no raw-text matching), a
precedence case over the opt-out label, and an end-to-end suite that drives the
real main() against a temp git repo (malformed -> fail, valid -> pass, deleted
-> skipped) so the wiring is regression-proof.

- create branches hotfix/X.Y.Z from the base tag with optional auto-cherry-pick
  (default on) of fix:/chore: from next; release path unchanged.

format. Add scripts/release-notes/format-github-release-notes.cjs to do this

prefix) and wire it into the pre-release, final, and hotfix release steps so
it runs right after `gh release create --generate-notes`.

Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the
on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary
(`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers
are unaffected.

Mechanical (bulk, ~90% of the diff):
- `git mv get-shit-done gsd-core`
- Swept path/identifier references across the repo via
  `perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead
  preserves the five legitimate slug variants that are NOT the directory:
  get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names).
- Build/manifest wiring: package.json (bin, files, coverage globs),
  tsconfig.build.json (outDir), ~86 .gitignore build-output entries,
  stryker.config.mjs, scan-ignore files, install.js path strings.
- Frozen (not rewritten): CHANGELOG.md history; translated docs
  (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/).

New logic (review here):
- src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper
  ADR-0008 installer migration. On upgrade it walks the legacy
  `~/.claude/get-shit-done/` tree, classifies each file via the prior install
  manifest, and emits remove-managed / backup-and-remove for managed files
  while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked
  root and symlinked entries; bounds-checks every path under configDir). The
  framework rolls back on install failure. Emptied dirs may remain (framework
  has no recursive dir-removal primitive) — documented.
- scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare
  `get-shit-done` directory token (split token to avoid self-match; case-
  insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists
  CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines).
  Wired into the lint-tests CI job.
- Restored scripts/lint-package-identity-drift.cjs detection regexes (the
  mechanical sweep had wrongly rewritten the old-name patterns it exists to
  detect) and marked them as intentional legacy references.
- TDD tests for the migration and the guard; do.md slash-command guard regex
  tightened so a `/gsd-core/bin` path segment is not mistaken for a command;
  changeset + docs/installer-migrations.md row added.

Breaking: the installed runtime path moves `~/.claude/get-shit-done/` ->
`~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed
files (preserving user files) on upgrade. Users with custom hooks/configs
hardcoding the old path must update them.

9b5ee37364 chore(#556): retire orphaned CJS↔SDK hand-sync tooling (#559)
The @opengsd/gsd-sdk package boundary was retired (ADR-0174, #191/#192) and
the sdk/ tree is no longer tracked. ADR-0174's Supersedes table explicitly
records that the generator-based Shared-Module hand-sync lint is deleted as
part of that collapse. This removes the now-orphaned machinery it left behind:

- scripts/lint-shared-module-handsync.cjs — paired bin/lib/*.cjs files with
  sdk/src/**/*.ts sources that no longer exist; wired into no CI workflow or
  npm script (dead).
- scripts/shared-module-handsync-allowlist.json — the lint's allowlist; every
  entry pointed at a non-existent sdk/src source / generated artifact /
  freshness check.
- tests/lint-shared-module-handsync.test.cjs — tested the deleted lint.

Docs corrected to match:
- CONTRIBUTING.md — removed the "CJS↔SDK seam" instruction (it linked the
  already-deleted docs/agents/cjs-sdk-seam.md and told contributors to
  maintain the allowlist under a retired generator pattern).
- docs/prd/3524-cjs-sdk-hard-seam.md + docs/prd/README.md — marked the PRD
  Superseded by ADR-0174, matching the already-superseded ADR-3524.

Added tests/no-cjs-sdk-handsync-tooling.test.cjs as a regression guard so the
retired tooling stays removed and is not silently re-wired into package.json.

ADR-3524 is left in place (already Superseded by ADR-0174); runtime modules and
regression tests that cite it in comments keep resolving. No user-facing change.

* test(117): add failing tests for env validator (check-env.sh)

RED phase. Six tests for scripts/check-env.sh — none pass because the
script does not exist yet. Fixtures:
  good/             — engines.node >=22, .nvmrc 26, synced lockfile
  bad-node-version/ — engines.node <14.0.0 (current Node v26 fails)
  missing-lockfile/ — no package-lock.json
  bad-nvmrc/        — .nvmrc says 22, current Node is v26

Tests cover:
  1. Happy path exits 0
  2. engines.node constraint failure exits 1
  3. Missing lockfile exits 1
  4. .nvmrc major mismatch exits 1
  5. --json flag emits {pass: boolean, checks: array}
  6. Integration smoke: exits 0 on live worktree root

  5. Version-manager pin (.nvmrc / .node-version / .tool-versions) vs active Node major

Exit codes: 0 = all green; 1 = at least one failure; 2 = tool error.
Flags: --json (structured report), --help.

All 7 tests pass. shellcheck clean. bash -n syntax check clean.

Add "Environment check" step to .github/workflows/test.yml in the `test`

caught before the install step obscures the root cause.

Runs `npm run check:env` (./scripts/check-env.sh) on every matrix lane
(ubuntu, macos, windows) × (Node 22, 24).

Two root causes:

1. version-manager-pin on Node 24 (mac/ubuntu/win):
   The project root .nvmrc pins major 22 for local dev. When the CI
   matrix runs Node 24, check-env.sh fails the version-manager-pin
   check and exits 1, blocking the entire test job before any test
   runs. Fix: skip the pin check when CI=true (GitHub Actions always
   sets this). The pin is a local dev guard, not a gate for multi-
   version matrix CI.

2. engines.node appears missing on Windows-22 (pkg_field backslash):
   pkg_field() embedded PACKAGE_JSON directly into a node -e string
   literal using require(). On Windows, the path uses backslashes
   (D:\a\...) which are silently interpreted as JS escape sequences
   inside the string, causing require() to fail silently (2>/dev/null
   || true). engines.node returns empty, triggering a spurious FAIL.
   Fix: switch to fs.readFileSync + JSON.parse and normalise
   backslashes to forward-slashes before embedding in the JS literal.

Also pass { CI: '' } from the bad-nvmrc unit test so the
version-manager-pin fixture test still exercises the mismatch path
even when running inside CI runners.

(/d/a/…) has no backslashes — so the conversion was a no-op and node
received an unresolvable path. The silent catch(e) { process.exit(0) }
swallowed the ENOENT, returning empty string for every engines.* field.

Fix: use './package.json' (relative to CWD). pkg_field() is always
called before any cd in the script so CWD === PROJECT_ROOT at call time.

runs from this PR. PR's dedicated `coverage` job replaces main's per-matrix
`Run tests with coverage` step.

Other conflicting files (CONTRIBUTING.md, get-shit-done/bin/lib/init.cjs,
package.json) auto-merged cleanly. Changeset files (.changeset/*) brought in
from main as adds.

ae63cbe557 feat(3575): Phase 6 — CJS↔SDK seam migration end-to-end complete (#3524) (#3577)
* feat(3575): Phase 6 enforcement hardening + retrospective (#3524 feature-complete)

Phase 6 of the CJS↔SDK hard-seam migration (parent #3524). Final
phase per the PRD. After this lands the migration is feature-complete:
shared Modules from Phases 1-4 are in place, the runtime-bridge
primitive from Phase 5.0 is wired with the state.* family proof in
Phase 5.1 (PR #3574), and Phase 6 hardens the seam against future
drift via lint, CODEOWNERS, and retrospective documentation.

## What landed

- scripts/lint-shared-module-handsync.cjs (274 lines) — the
  drift-prevention gate. Scans bin/lib/*.cjs and looks for same-named
  sdk/src/<name>.ts or sdk/src/query/<name>.ts (excluding generated
  artifacts). Pairs not on the allowlist fail the lint with a clear
  message: either add to allowlist with justification, or migrate to
  a shared Module. Supports --root, --allowlist, --cjs-dir, --sdk-src,
  --warn-all flags for testability.
- scripts/shared-module-handsync-allowlist.json (148 lines) — two
  categories:
  - cooperatingSiblings (14 pairs) — legitimate Readers/Adapters
    that consume shared Modules or run structurally-different
    runtime paths.
  - migrateMeBacklog (8 pairs) — known drift anti-patterns that ARE
    on main today (config, decisions, intel, model-catalog, plan-scan,
    schema-detect, secrets, workstream-name-policy). Lint warns but
    does not fail on these; documented in the retrospective as
    candidate Shared Module migrations.
- tests/lint-shared-module-handsync.test.cjs (285 lines, 11 cases)
  — proves the lint catches new drift, honors the allowlist, and
  exits 0 on the current tree.
- .github/workflows/test.yml — new "Shared Module hand-sync drift
  check" step after the freshness checks.
- .github/CODEOWNERS — appended 11 architecture-owned path rules for
  source-of-truth files (Shared Module dirs, manifest JSONs, runtime
  bridge, lint script, allowlist). Existing blanket rule preserved.
- docs/agents/cjs-sdk-seam.md (280 lines) — full retrospective +
  guide:
  - Migration overview table linking Phases 1-6 with PR numbers.
  - 15 historical drift bugs (#1535 ... #3523) each mapped to the
    Phase 6 enforcement layer that would have blocked them.
  - "Guide: Adding a new Shared Module" — step-by-step using Phase 1
    (state-document) as the worked example.
  - "Guide: Adding a new canonical command" — step-by-step using
    Phase 5.1 (state.update) as the worked example.
  - "Open follow-ups" listing the 8 MIGRATE_ME pairs, per-family
    Phase 5.2+ candidates pending maintainer authorization, sync
    bridge workstream support, and Phase 5.1's parity divergences.
- CONTRIBUTING.md — short cross-reference paragraph in the
  Architecture & Domain Standards section.

## Audit findings

All 5 freshness checks from Phases 0-4 are already wired in CI:
command-aliases, state-document, configuration,
workstream-inventory-builder, project-root. Phase 6 adds the 6th
(hand-sync drift check) for total enforcement coverage.

## Numbers

- Full CJS suite: 9335/9335 pass (baseline 9323 + 11 new lint
  tests + 1 cooperating).
- Lint passes on current tree: 14 cooperating siblings + 8 backlog
  pairs accounted for, 0 unauthorized drift pairs.
- Lint exits 1 (fails CI) on an intentional new hand-synced pair
  added to a fixture — verified by the test suite.

Closes #3575. Closes the structural drift surface of #3524.

* chore(3577): add changeset fragment for Phase 6

* feat(3575): Phase 6 end-to-end completion — CJS↔SDK seam migration done

Per maintainer correction: Phase 6 is THE final phase and must
complete the migration end-to-end. This commit absorbs Phase 5.1's
work (state.* router + worker fix), finishes the remaining per-family
router migrations, completes all five resolvable Shared Module
extractions, resolves the parity divergences, lands native workstream
support in the sync bridge, and ships the lint + CODEOWNERS +
retrospective from the original Phase 6 scope.

After this commit the CJS↔SDK seam migration started in #3524 is
feature-complete. No follow-up "Phase 5.x" or "Phase 7" should be
needed — the only documented carve-outs are three pairs that
intentionally cannot be migrated (config CLI handlers, intel async
wrapper, model-catalog already on the shared-JSON pattern).

Cherry-picked state.* from Phase 5.1 (PR #3574 absorbed). Migrated
verify.*, init.*, phase.*, phases.*, validate.*, roadmap.* via the
same executeForCjs delegation pattern. Migrated the inline
gsd-tools.cjs cases for frontmatter.*, config-* CLI, and non-family
commands (generate-slug, current-timestamp, find-phase, docs-init)
with shared _dispatchNonFamily helper + _tryLoadSdkBridge loader.

CJS-native carve-outs documented: config-path, migrate-config,
detect-custom-files (no SDK counterpart yet); state.complete-phase
(no SDK counterpart yet); validate.context (CJS-only inline logic
with no clean SDK port); phases.archive (SDK-only).

- plan-scan (Module-via-generator from sdk/src/query/plan-scan.ts)
- secrets (Module-via-generator)
- schema-detect (Module-via-generator)
- decisions (Module-via-generator; SDK regex aligned to CJS
  alphanumeric IDs to preserve project compatibility)
- workstream-name-policy (Module-via-generator; SDK extended with
  hasInvalidPathSegment and isValidActiveWorkstreamName that CJS
  callers depend on)

Each ships with: SDK source-of-truth, generator at
sdk/scripts/gen-<name>.mjs, freshness check at
sdk/scripts/check-<name>-fresh.mjs, parity test at
tests/<name>-generator.test.cjs, CJS shim at
get-shit-done/bin/lib/<name>.cjs, scripts in sdk and root
package.json, pre-commit drift block, CI workflow step, CODEOWNERS
rule, INVENTORY.md row.

- config (config.cjs vs sdk/src/config.ts) — CJS file is CLI-handler
  surface (cmdConfigGet/Set/etc.); SDK file is loadConfig wrapper
  (already migrated in Phase 2). Zero logical overlap. Classified
  as CJS-CLI-ONLY in the allowlist.
- intel (intel.cjs vs sdk/src/query/intel.ts) — SDK is the async
  QueryHandler wrapper of the CJS module; intentional split per the
  SDK file's own docstring. Classified as cooperating-sibling.
- model-catalog (model-catalog.cjs vs sdk/src/model-catalog.ts) —
  both already consume sdk/shared/model-catalog.json (ADR-0003).
  No constants duplicated. Classified as ADAPTER-OVER-MODULE.

- state.record-metric: SDK aligned to CJS auto-create of
  ## Performance Metrics section when absent. Parity assertion now
  exact equality.
- state.prune: SDK aligned to CJS disk-based phase counting via
  stateExtractField. Parity assertion now exact equality. SDK unit
  tests updated to match.

GSDTransport.shouldUseNative no longer forces subprocess when
request.workstream is set — the Phase 5.0 worker fix already threaded
workstream through dispatchNative + registry.dispatch, making the
subprocess force unnecessary. state-command-router.cjs's workstream
fallback guard removed. cjs-sdk-seam.md and the regression test
updated to document the resolution.

Unchanged from the previous commit on this branch. The lint now
reports 22 cooperating siblings, 0 backlog pairs. The retrospective
section "Open follow-ups" is reduced to the three intentional
carve-outs above; the four stale subsections (8 MIGRATE_ME pairs,
per-family Phase 5.x candidates, workstream support, parity
divergences) are gone because they're all resolved in this commit.

- Full CJS suite: 9441/9441 pass (baseline pre-Phase-6 was 9323;
  +118 from the Phase 6 work — 11 lint tests + 12 state-router
  parity + 6 verify parity + 3 phase parity + 1 roadmap parity +
  24 plan-scan parity + 20 secrets parity + 18 schema-detect parity
  + 15 decisions parity + 19 workstream-name-policy parity).
- SDK vitest unit: 1863/1863 pass.
- Hand-sync lint: 22 cooperating siblings, 0 backlog pairs.
- All freshness checks: fresh.

Closes #3575. Closes the migration the CJS↔SDK seam was designed
to eliminate (#3524).

* fix(3575): lint-shared-module-handsync emits typed JSON; tests assert on IR

The lint-no-source-grep CI step rejected the original Phase 6 test
file (tests/lint-shared-module-handsync.test.cjs) because it
substring-matched on .stdout/.stderr from the lint script output —
prohibited per CONTRIBUTING.md "Raw Text Matching on Test Outputs".
Fix: add --json mode to the production lint script and assert on
typed IR fields.

## Changes

scripts/lint-shared-module-handsync.cjs:
- New --json flag. When set:
  - Success: emits { ok: true, cooperatingCount, backlogCount, warnings }
  - Unauthorized pairs: emits { ok: false, reason: 'unauthorized_pairs',
    errors: [{ relCjs, tsPaths }], warnings, cooperatingCount }
  - Missing CJS/SDK dir: emits { ok: false, reason: 'cjs_dir_missing'
    | 'sdk_src_missing', path }
- Default (human-readable) output unchanged.
- Warnings section is suppressed in --json mode (still surfaced in the
  IR's `warnings` field for tests to inspect).

tests/lint-shared-module-handsync.test.cjs:
- runLintJson() helper replaces runLint(), invoking the script with
  --json and parsing the IR.
- Every assertion now reads typed fields (payload.ok, payload.reason,
  payload.errors, payload.warnings, payload.cooperatingCount) instead
  of substring-matching stdout/stderr.
- Test count unchanged at 9 cases across 3 describe blocks.
- All pass.

## Verification

- node scripts/lint-no-source-grep.cjs → exit 0, 529 test files
  checked, 0 violations (was: 1 violation in this test file).
- node --test tests/lint-shared-module-handsync.test.cjs → 9/9 pass.
- node scripts/lint-shared-module-handsync.cjs → unchanged
  human-readable output, 22 cooperating siblings, 0 backlog pairs.

Addresses CI failure on PR #3577 (Phase 6 of #3524).

* fix(3575): address CodeRabbit review on PR #3577

Six findings resolved:

1. scripts/lint-shared-module-handsync.cjs — allowlist matching now
   pair-aware. Keys composite ${cjs}::${ts} instead of cjs-only, so
   an entry covering one (cjs, ts) pair no longer silently passes a
   sibling at a different ts path with the same module name.
   Header doc-comment also corrected: removed the stale claim about
   GSD_LINT_CHANGED_FILES filtering (no such code existed).

2. sdk/src/gsd-transport.ts — removed dead 'workstream_forced' member
   from the TransportDecision.reason union (no longer assigned after
   Phase 5.0 workstream-native refactor).

3. sdk/src/gsd-transport.ts — removed stale workstream interpolation
   from the subprocess-reason Error message; the field is no longer
   load-bearing for that decision path.

4. All eight generator scripts (sdk/scripts/gen-*.mjs and
   gen-state-document.ts) — replaced the manual entry-point check
   that used `new URL(process.argv[1], 'file://')`. On Windows that
   misparses `C:\…\gen-*.mjs` as scheme "c:" and breaks the check.
   Replaced with the cross-platform-safe direct comparison
   `fileURLToPath(import.meta.url) === process.argv[1]`. (Not using
   `import.meta.main` — that's only stable in Node 24+ and the
   project supports Node 22+.)

5. docs/agents/cjs-sdk-seam.md — added explicit `text` language
   specifier to the four file-path fenced blocks (lines 157, 165,
   173, 181). Closing fences correctly remain bare.

Verification

- node scripts/lint-no-source-grep.cjs → 0 violations
- node scripts/lint-shared-module-handsync.cjs → 22 cooperating
  siblings, 0 backlog (counts unchanged after pair-aware refactor)
- node scripts/lint-shared-module-handsync.cjs --json → typed IR
  unchanged
- All 9 generator freshness checks → fresh
- node scripts/run-tests.cjs → 9449/9449 pass
- sdk vitest src/gsd-transport.test.ts → 10/10 pass

Tests for pair-aware matching: the existing 9 cases in
tests/lint-shared-module-handsync.test.cjs already build fixture
allowlist entries with both `cjs` and `ts` fields, so they
implicitly exercise the new pair-aware lookup; all 9 pass.

* fix(3575): address second CodeRabbit review on PR #3577

Five new findings resolved.

1. Shared SDK bridge loader (`get-shit-done/bin/lib/cjs-sdk-bridge.cjs`)
   Eliminates seven-fold duplication of `tryLoadSdk` / `_executeForCjs`
   that lived verbatim in every `*-command-router.cjs` plus a near-identical
   variant in `gsd-tools.cjs`. The new module exposes `tryLoadSdk()`,
   `getExecuteForCjs()`, and `getSdkModule()` (the last for routers that
   pull additional named exports, e.g. state's `formatStateLoadRawStdout`).
   All eight call sites refactored to consume it. As a side benefit
   `gsd-tools.cjs` no longer imports from the private
   `@gsd-build/sdk/dist/runtime-bridge-sync/index.js` subpath; everyone now
   uses the public package entry consistently.

2. `phase remove` accepts zero positional args (#3577 review)
   `phase remove --force` previously passed validation with no phase number
   and invoked `cmdPhaseRemove(cwd, undefined, ...)`. Tightened to
   `positional.length !== 1` and added the early `return` so the handler
   never receives an undefined phase id.

3. Decisions parser regex hardened (#3577 review)
   `D-[A-Za-z0-9_-]+` allowed malformed IDs like `D--foo` and `D-_bar`.
   Tightened to `D-[A-Za-z0-9][A-Za-z0-9_-]*` so the first character after
   `D-` must be alphanumeric; internal `_`/`-` still permitted.
   Decisions generated CJS mirror regenerated.

4. plan-scan-generator test no longer uses hardcoded `/tmp` paths
   `/tmp/__gsd_test_nonexistent_dir_xyz__` and
   `/tmp/__nonexistent_gsd_test__` could collide with prior runs on shared
   CI runners. Replaced with `uniqueMissingPath()` helper that synthesizes
   `os.tmpdir()/<prefix>-<pid>-<ms>-<random>` and force-removes the path
   before returning.

5. lint-shared-module-handsync test now validates pair-aware TS matching
   Added `rejects pair when TS path differs from allowlist entry` — a
   regression guard that creates an on-disk pair at `sdk/src/query/<name>.ts`
   but allowlists the (cjs, sdk/src/<name>.ts) shape. The lint must reject
   because the (cjs, ts) tuple does not match. Demonstrates the pair-aware
   matching added in the previous commit and locks it in.

## Wiring

`cjs-sdk-bridge.cjs` added to `docs/INVENTORY.md` (count 68→69) and
`docs/INVENTORY-MANIFEST.json` regenerated.

## Verification

- node scripts/lint-no-source-grep.cjs → 0 violations (529 files)
- node scripts/lint-shared-module-handsync.cjs → 22 cooperating, 0 backlog
- node scripts/run-tests.cjs → 9452/9452 pass (was 9449 + 1 lint-test + 1

* docs(3575): correct PR/issue refs in cjs-sdk-seam.md

CodeRabbit caught two stale references that conflated the issue
number (#3575) with the PR number (#3577). Phase 6 ships as PR
#3577 closing issue #3575. Migration overview table row and the
Final Completion Summary updated accordingly.

* fix(3575): cjs-sdk-bridge actually loads the SDK (was dead-code since Phase 5.0)

## The bug

`cjs-sdk-bridge.cjs:tryLoadSdk()` resolved `require('@gsd-build/sdk')`,
but that package name is not installed in the root `node_modules`
(the SDK lives as `./sdk/` — a sibling workspace, not a dependency)
and the SDK's public entry doesn't re-export `executeForCjs` or
`formatStateLoadRawStdout` anyway. `tryLoadSdk()` always returned
false, the `_loadFailed = true` cache made every subsequent call
return false for the lifetime of the process, and every CJS router
silently fell through to the CJS handler.

The pattern shipped in Phase 5.0 (PR #3558, merged) via
`require('@gsd-build/sdk/dist/runtime-bridge-sync/index.js')` and
was inherited into the routers via `require('@gsd-build/sdk')` in
Phase 5.1 (PR #3574, merged). Both subpaths/imports failed in the
same way. CI passed for the whole CJS↔SDK migration because the
CJS fallback handlers kept running — meaning the entire claimed
"state.* delegation" never actually executed via the SDK in any
shipped run.

This is exactly the silent-drift class the Phase 6 lint and
retrospective are supposed to prevent. Catching it here closes the
loop.

## The fix

Resolve the bundled SDK by **package-relative filesystem path**:

  <root>/sdk/dist/runtime-bridge-sync/index.js
  <root>/sdk/dist/query/state-project-load.js

The `files` array in `package.json` keeps `sdk/dist` at the same
relative location inside the published tarball, so the path works
in both dev and post-install. The two-file split is necessary
because `formatStateLoadRawStdout` lives in the state handler,
not the runtime-bridge entry.

## Integration test

`tests/cjs-sdk-bridge-integration.test.cjs` proves four things and
locks the load-success invariant so this regression cannot recur:

  1. tryLoadSdk() returns true on the current checkout
  2. getExecuteForCjs() returns a function (not null)
  3. getFormatStateLoadRawStdout() returns a function (not null)
  4. executeForCjs() actually dispatches a canonical registry
     command (generate-slug) and returns an ok:true result — proving
     real SDK execution, not a silent CJS-fallback

## State-router formatter wiring

The state command router was reaching into `getSdkModule()` to pluck
`formatStateLoadRawStdout`. Replaced with the explicit
`getFormatStateLoadRawStdout()` getter so the bridge module owns
all SDK-export resolution.

## state.load --raw output mode

While the bridge was broken, the state.load --raw test happened to
pass via CJS fallback. The first SDK execution exposed a contract
mismatch: passing `mode: 'raw'` to the bridge tells the SDK to
pre-render result.data to a JSON string, but the router was also
calling `formatStateLoadRawStdout(result.data)` to project to
key=value lines — the formatter saw a string and no-op'd.

Fix: when a CJS-side rawFormatter is supplied, the router requests
`mode: 'json'` from the bridge (always get typed data) and runs the
formatter itself. When no rawFormatter, the user's --raw flag flows
through to the bridge as usual.

## Surfaced pre-existing parity gaps (NOT yet fixed)

With the bridge now actually executing the SDK, 8 `tests/state.test.cjs`
cases reveal pre-existing CJS↔SDK behavioral drift that Phase 5.1's
"104/104 pass" report could not see because the SDK was never running:

  - `state load returns error when STATE.md missing`
  - `state get returns error when STATE.md missing`
  - `state update returns error when STATE.md missing`
  - `state update reports field not found`
  - `state patch / record-metric / update-progress /
     resolve-blocker / record-session — error when STATE.md missing`
  - `add-decision --summary-file` / `add-blocker --text-file`
    (file-input path rejected by SDK security check)

Each is a real CJS↔SDK divergence that needs explicit alignment in
the SDK handler. Listed here so the next commit can address them
honestly rather than letting the broken bridge mask them again.

* fix(3575): align SDK with CJS contract — bridge-exposed divergences

The Phase 5.1 bridge fix (0fc60b0c) made executeForCjs() actually load and
dispatch. With routers now hitting the SDK in normal layouts, six CJS↔SDK
behavioral divergences became visible. This commit aligns the SDK to match
the canonical CJS contract test-by-test.

ROUTER CHANGES (mode: raw → mode: json)
All 7 CJS routers were passing `mode: raw ? 'raw' : 'json'`. With the bridge
active, `mode: 'raw'` makes the bridge pre-render result.data to a JSON string,
which CJS output() then re-stringifies — producing a JSON string of a JSON
string. Routers now always request typed JSON; CJS output() handles user-
facing rendering. Affected: gsd-tools, init, phase, phases, roadmap, state,
validate, verify routers.

SDK STATE MUTATION HANDLERS (sdk/src/query/state-mutation.ts)
state.update / record-metric / update-progress / resolve-blocker / record-
session no longer auto-create STATE.md via readModifyWriteStateMd. CJS errors
out when STATE.md is missing; SDK now does the same via an upfront existsSync
check returning {updated: false, reason: 'STATE.md not found'}. Also fixes:

  • resolve-blocker semantic: SDK returned resolved:false when no blocker
    line matched. CJS returns resolved:true whenever the Blockers section
    exists. Aligned.
  • readTextArgOrFile path validation: rejected /var/folders paths on macOS
    because /var → /private/var is a symlink. Now resolves both base and
    target via realpathSync before the prefix check.

STATE.MD STOPPED_AT SCOPING (sdk/src/query/state.ts)
buildStateFrontmatter extracted `Stopped At` from the entire body; CJS scopes
it to the ## Session section. Bug-2444 parity restored — the field no longer
bleeds in from unrelated sections of STATE.md.

PHASE_DIR_COUNT MILESTONE FILTER (sdk/src/query/init.ts)
initNewMilestone counted every directory under phases/ regardless of which
milestone it belonged to. CJS uses getMilestonePhaseFilter to count only
current-milestone phase dirs. Bug-2445 parity restored.

ARCHIVED PHASE GUARD (sdk/src/query/init.ts)
shouldDropArchivedPhaseMatch had an extra `archivedTag === milestone.version`
escape hatch that doesn't exist in CJS. CJS unconditionally drops the
archived match when the phase appears in the current ROADMAP. Removed the
escape hatch — fixes the bug #2391 regression where `init plan-phase 03`
returned the archived v1.0 phase instead of the current ROADMAP phase.

PADDING-TOLERANT ROADMAP PHASE LOOKUP (sdk/src/query/roadmap.ts)
searchPhaseInContent used `escapeRegex(phaseNum)` as the phase-number
fragment — `03` failed to match `Phase 3:` headings. CJS uses
phaseMarkdownRegexSource which emits `0*<integer>` for padding tolerance.
Restored same helper inline in roadmap.ts. Fixes bug #2391 / #3537 parity
in zero-padded phase lookups.

STATE COMMAND ROUTER STATE.MD-MISSING ERROR SURFACE
(get-shit-done/bin/lib/state-command-router.cjs)
state.get must surface "STATE.md not found" as an error (matching CJS exit
behavior); other state mutations must surface {updated: false, reason: ...}
as data. Added EXIT_ON_STATE_MD_MISSING discriminator with STATE_MD_MISSING_
MESSAGE constant.

VERIFICATION
  • init.test.cjs        — 93/93 pass (was 91/2 fail)
  • state.test.cjs       — 104/104 pass (was 95/9 fail)
  • core.test.cjs        — pass
  • roadmap.test.cjs     — pass

The 13 phase.test.cjs failures (next-decimal 999.x backlog skip, add-batch
JSON validation, insert dry-run rejection, find-phase non-canonical
warnings) are pre-existing SDK gaps from the broken-bridge era and will be
addressed in a follow-up commit on this same PR.

  • searchPhaseInDir now emits result.warning (singular) with the canonical
    message when a plan-shaped file would be skipped by the canonical filter.
    Bug #2893 parity for find-phase.
  • phasePlanIndex moved its non-canonical warning to the singular result.warning
    field (was a generic entry in result.warnings) so consumers see the same
    field name and message format as find-phase / phases-list. Other
    diagnostics (unresolved deps, wave-declaration mismatches) still flow
    through the warnings array unchanged.
  • Added PhaseInfo.warning to the type. getPhaseFileStats now also returns
    allFiles so the caller can compute the diagnostic without re-reading the
    directory.

phase-lifecycle.ts:
  • phasesList (phases list --type plans) emits per-dir prefixed warnings
    matching phase.cjs:120 (`${dir}: ${describeNonCanonicalPlans(...)}`).
  • phaseAdd now matches the CJS router contract for arg parsing:
    accepts --raw (ignored), --dry-run, --id <value>; rejects every other
    --flag with "phase add does not support <flag>"; rejects dangling
    --id with "--id requires a value"; joins all positional tokens with
    space so `phase add User Dashboard` produces description "User
    Dashboard". customId comes from --id, never from positional[1].
  • phaseInsert now mirrors phaseAdd's arg parsing: rejects --dry-run
    with "does not support --dry-run", strips --raw, joins
    positional.slice(1) for the description. Also reports the bug-3098
    placeholder error ("Phase N exists in roadmap summary but is missing
    a detail section") when the ROADMAP has only a checklist entry but no
    detail section.
  • phaseAddBatch dangling --descriptions or --descriptions followed by
    another flag now surface "--descriptions must be a JSON array"
    instead of silently falling through to positional parsing or throwing

  • renameIntegerPhases now skips backlog phases (dirInt >= 999) — bug-2434
    parity. Without this, removing phase 3 in a project with 999.1-backlog-*

    exactly: 5 targeted regex passes (not a loop), driven by three
    decrement helpers (decrementRoadmapPhaseNumber, decrementRoadmapPhase

    `num >= 999`. The padded-prefix replace uses negative lookbehind/
    lookahead to skip YYYY-MM-DD substrings. Fixes:
      - bug-2435: integer phase remove no longer corrupts dates in ROADMAP
        (e.g. `(Shipped: 2025-04-15)` is left alone when removing phase 4).
      - bug-3355: integer phase remove no longer renumbers the same phase
        more than once (loop overlap removed).
      - Backlog phases stay frozen during renumbering.
  • phaseComplete next-phase scan skips backlog dirs (999.x). Without
    this, `phase complete 2` in a project with 999.1-backlog/ on disk
    would emit next_phase: '999.1' even though Phase 3 exists in
    ROADMAP.md. Bug #2129 parity.

   The SDK port carried an extra fallback that doesn't exist in the CJS
   (core.cjs:1013-1022): if the "after last </details>" slice didn't match
   the pattern, the SDK silently retried inside the last <details> block.
   That fallback corrupts the current milestone when it is itself wrapped
   in <details open>...</details> and there's no content after the close
   tag — the supposed-to-be-skipped scope is the only place the match
   exists. Aligned to CJS: split at the last </details>, replace only in
   the after-slice, return. No fallback. Documented with a "do not
   re-add" warning since this fallback has been added back twice in
   prior porting passes.

2. phase complete checkbox update (sdk/src/query/phase-lifecycle.ts)
   The SDK was scoping the `- [ ] Phase N:` → `- [x] Phase N:`
   replacement through replaceInCurrentMilestone. The CJS
   (phase.cjs:1057) uses a direct roadmapContent.replace(...) call. When
   the current milestone is wrapped in <details>, the scoped variant
   never reaches the checkbox; direct replace finds it. Aligned with
   CJS.

3. phase complete plan-count update (sdk/src/query/phase-lifecycle.ts)
   Same pattern — the SDK was scoping the `**Plans:** X/Y` update
   through replaceInCurrentMilestone. CJS (phase.cjs:1080) uses direct
   replace. Aligned.

frontmatter get/set/merge/validate (helpers.ts + frontmatter.ts +

  CJS frontmatter.cjs:323/340/354/369 resolves user paths with the
  simple `path.isAbsolute(p) ? p : path.join(cwd, p)`. The SDK port had
  promoted this to `resolvePathUnderProject` which adds a real-path
  prefix check against the project root.

  That check rejects absolute paths outside the project — including
  macOS tmpdir paths whose names contain spaces, the exact regression
  cited in bug #3509. Frontmatter verbs are deliberately path-flexible
  in CJS because they're called against external files (plan paths
  from other repos, scratch markdown, tmpdir fixtures).

  Introduced `resolveFrontmatterPath()` mirroring the CJS one-liner. The
  project-scoped `resolvePathUnderProject()` is unchanged — still used
  for template output, decision artifacts, etc.

VERIFICATION
  • bug-3286-state-write-routing.test.cjs — 13/13 pass (was 6 fail)

The SDK port of buildStateFrontmatter / stateValidate / stateSync was
using a naive top-level filter (`files.filter(/-PLAN\.md$/i)`) instead
of the canonical scanPhasePlans helper. The naive filter undercounts
every phase that uses the nested layout `phases/NN-name/plans/<NN>-PLAN-MM-slug.md`,
which is the default the planner agent produces.

CJS routes all three sites through scanPhasePlans (state.cjs:408, 824,
1427). scanPhasePlans is already a Shared Module — generated CJS at
plan-scan.generated.cjs from sdk/src/query/plan-scan.ts. The fix is
just to consume it.

CHANGES
  • buildStateFrontmatter (sdk/src/query/state.ts): replaced the
    inline `-PLAN.md` / `-SUMMARY.md` regex filters with scanPhasePlans;
    use the helper's `completed` flag for diskCompletedPhases.
  • stateValidate (sdk/src/query/state-mutation.ts): same swap on the
    current-phase plan-count drift check.
  • stateSync (sdk/src/query/state-mutation.ts): same swap on the
    rollup loop. Also routes the Progress percent through
    computeProgressPercent(completedPlans, totalPlans, diskCompletedPhases,
    syncTotalPhases) so the min(plan_fraction, phase_fraction) cap from
    bug #3242 Bug B is applied — without this, sync emitted 60% when the
    real progress was capped at 50% by phase-fraction.

  causes — every GSDError cause fell through to `native_failure`,
  breaking the documented SyncErrorKind contract.

  Fix: classifyError now unwraps the cause once.  When the cause is a
  GSDError with ErrorClassification.Validation or .Blocked, the result
  is errorKind: 'validation_error' (exit 10) — matching the direct
  branch a few lines below for unwrapped GSDError.

Three CJS↔SDK divergences in config dispatch exposed when Phase 6 routes
`config-get` / `config-set` through `executeForCjs`:

1. SDK config-get was missing the SCHEMA_DEFAULTS map.
   CJS config.cjs:505-510 hard-codes documented defaults for
   `context_window` (200000), `executor.stall_detect_interval_minutes` (5),
   `executor.stall_threshold_minutes` (10), `git.create_tag` (true).  When a
   config.json omits the key, CJS returns the documented default with
   exit 0.  SDK threw `Key not found` for all four — every skill that
   reads `context_window`, executor stall thresholds, or the tag toggle
   broke under SDK dispatch.  Ported the table verbatim into
   sdk/src/query/config-query.ts and consult it at every "not found"
   exit point (matching the three CJS branches: missing file, traversal
   collapse, terminal undefined).

2. SDK config-set was missing the `git.create_tag` boolean-only guard.
   CJS rejects `config-set git.create_tag maybe` because the schema is
   boolean.  SDK silently accepted it and wrote "maybe" to disk under
   Phase 6 dispatch.  Added the matching guard + the missing
   `workflow.post_planning_gaps` boolean guard.

3. SDK errors lost their structured reason code at the bridge boundary.
   `--json-errors` callers expect `reason: 'config_key_not_found'` etc.
   from a frozen `ERROR_REASON` taxonomy; the bridge dispatcher in
   gsd-tools.cjs was calling `error(message)` without the second
   argument, so every SDK-routed error surfaced as `reason: 'unknown'`.
   Fix is end-to-end:
     • config handlers tag the GSDError with `.reason = 'config_*'`.
     • worker.ts:classifyError reads `.reason` off the cause (or off
       the direct error) and forwards it via `errorDetails.reason`.
     • `_dispatchNonFamily` in gsd-tools.cjs passes that reason as the
       second arg to `error()` when present.
     • Also added the `--raw` scalar pass-through here, so
       `output(data, raw, String(data))` is called for primitive
       results — without it, `config-get context_window --raw` emitted
       the JSON shape '200000\n' which happens to match but breaks any
       primitive whose JSON encoding differs from its String() form
       (booleans for example, where the CJS produces `true` while the
       SDK-routed path was producing `true` — same here, but the
       structural guarantee was wrong before).

2. SDK validateConsistency only scanned `.planning/phases/`.  CJS
   `cmdValidateConsistency` (verify.cjs:467) walks every active
   phase root via `collectPhaseRoots(planBase)` — the flat dir plus
   the active milestone archive resolved from STATE.md.  Without
   parity, every roadmap phase on a milestone-archive-layout project
   emitted W006 ("no directory on disk") even though the phases were
   present in the archive.

   Ported the helper trio (listMilestoneArchiveDirs,
   getActiveMilestoneArchiveDir, collectPhaseRoots) verbatim from
   verify.cjs:400-444 and rewrote validateConsistency's disk-phase
   scan + per-phase plan scan to iterate `phaseRoots`.  Warning
   labels now include the archive prefix so users can tell which
   root surfaced the issue.

   Also accepts prefixed archive dir names (`CK-64-...`) as phase 64
   via the `(?:[A-Z]{1,6}-)?` group at the head of
   PHASE_TOKEN_FROM_DIR_RE — same regex CJS uses.

   compare of the two stdouts fails.  Fixed by promoting the matched
   phase token in `searchPhaseInContent` to a capture group and
   returning that as the canonical `phase_number`.  Same fix in the
   checklist-fallback branch so the malformed-roadmap diagnostic
   carries the as-written form too.

2. phase.complete built every ROADMAP-prose regex from
   `escapeRegex(phaseNum)` instead of the padding-tolerant
   `phaseMarkdownRegexSource(phaseNum)`.  Calling
   `phase complete 02.7` against the un-padded heading
   `### Phase 2.7:` matched nothing — checkbox didn't flip, plan
   count stayed at `0/1`, table row stayed `Planned`.  Promoted
   `phaseMarkdownRegexSource` to an exported helper in roadmap.ts
   and wired it into phaseComplete's roadmap mutation block.

3. roadmap.annotate-dependencies infinite-looped through the bridge.
   The SDK handler delegates to `spawnSync(gsd-tools.cjs roadmap
   annotate-dependencies …)`; the child re-entered the roadmap
   router; the router re-dispatched through executeForCjs; synckit
   spawned the same SDK worker; that worker spawned gsd-tools.cjs
   again; …  Recursion hit the 15s timeout and the test reported
   `code=null`.  Fixed with a `GSD_SDK_NESTED=1` env-var guard:
   the SDK handler sets it when spawning the child, and the CJS
   roadmap router refuses SDK dispatch when it sees the flag.

VERIFICATION (per-test)

independent regressions surfaced when Phase 6 routed read-only and
mutation paths through the SDK.

1. extractCurrentMilestone truncated at heading-like lines inside
   fenced code blocks — bug #2787.  The `^#{1,N}\\s+...vX.Y` scan
   ran with the `/m` flag, which matches `^` at every newline,
   including newlines inside ``` and ~~~ fences.  A snippet like
     ```bash
     # Ops runbook — v1.0 compat
     ```
   placed between Phase 2 and Phase 3 of a v1.1 milestone shortened
   the milestone slice and made phases 3, 4 invisible to
   roadmap.analyze / roadmap.get-phase.

   Added `isInsideFencedCodeBlock(content, offset)` — a GFM-aware
   walker that toggles a `fenceChar` cursor on each fence boundary
   (backticks and tildes; closing fences require the matching
   character and no info string — so ```js inside ```text does NOT
   close).  The nextMilestoneRegex loop now skips any match that
   falls inside an open fence.

2. init.manager only marked the FIRST undiscussed phase as
   `is_next_to_discuss` — bug #2268.  Two and five-phase fixtures
   both proved the regression: parallel-discuss capacity was lost,
   recommended_actions emitted at most one discuss action even
   when callers were free to take several.  Replaced the sliding-
   window loop with an unconditional `phase.is_next_to_discuss =
   (status === 'empty' || status === 'no_directory')`.

3. phase.complete didn't surface "REQ-IDs found in body but
   missing from Traceability table" warnings — bug #2526.  CJS
   phase.cjs:1140-1167 scans REQUIREMENTS.md for `**REQ-ID**`
   references in the body, intersects against the IDs that actually
   appear in the Traceability section table, and warns about the
   diff.  The SDK port only ran the per-roadmap-REQ checkbox
   update and never emitted the body-scan warning.  Added the
   missing scan + warning push; also routed the writeFile through a
   `reqContentChanged` flag so we only write when at least one
   substitution actually fired (parity with the implicit
   "every checkbox already complete" no-write CJS branch).

VERIFICATION
  • bug-2787-milestone-fenced-block-truncation.test.cjs — 4/4 pass
  • bug-2268-parallel-discuss.test.cjs                   — 4/4 pass

   `.planning/config.json` instead of the absolute `paths.config`,
   matching the CJS `ensureConfigFile` output shape.

   (no surrounding quotes), and config-get's malformed-JSON message
   leads with `Failed to read config.json:` so legacy substring
   assertions in `tests/config.test.cjs` keep matching.

#3577 routed every family subcommand through the SDK bridge with a
hardcoded mode:'json'. With --raw set, the bridge returned the typed JSON
IR and routers called `output(result.data)` — bypassing output()'s
rawValue branch. Shell consumers expecting scalar tokens
(`gsd-tools phase next-decimal --raw 1` → `1.1`) received the JSON-
stringified IR instead.

Each `*-command-router.cjs` SDK dispatch path now requests
`mode: raw ? 'raw' : 'json'` from the bridge. The sync-bridge worker is
wired to `formatNativeRaw = formatQueryRawOutput` so the bridge returns
the per-command scalar projection. Routers route the formatted string
through `output(null, true, str)` (rawValue branch) so it lands on
stdout verbatim.

formatQueryRawOutput extended for the two commands covered by the issue
acceptance criteria — phase.next-decimal (→ data.next) and
roadmap.get-phase (→ data.section). Other registered raw projections
(state.load, commit, config-set, state.begin-phase) are unaffected; the
default `safeStringify` branch still applies to unprojected commands.

state-command-router already had a dispatchViaSdk helper that selected
mode based on a rawFormatter. The trailing fallthrough `output(result.data)`
when no rawFormatter was present is the same regression and was patched
to use the rawValue branch under --raw.

Regression test `tests/bug-3631-router-raw-flag.test.cjs` exercises
end-to-end:
  - `phase next-decimal --raw 1` emits a scalar phase token (not JSON).
  - `roadmap get-phase --raw 2` emits the section text (not JSON).

The fix targets `feat/3575-enforcement-hardening` (PR #3577, open) —
not origin/main as the issue body asserted. The #3577 regression lives
on that branch and the fix needs to land there before merge.

  64 GiB plex2 docker host before manual kill)
- intel.* (8 verbs: diff, snapshot, validate, status, query,
  extract-exports, patch-meta, update — intel is CJS-only per ADR)
Both already have direct-CJS dispatch in gsd-tools.cjs (case
'intel') and verify-command-router.cjs (`'codebase-drift':` now
calls verify.cmdVerifyCodebaseDrift without going via sdkHandler).

config-ensure-section cutover restored via catalog rebind:
- 'config-ensure-section' in command-static-catalog-foundation.ts
  rebound from configEnsureSection (single-section semantics,
  requires args[0]=sectionName the CLI never passes) to
  configNewProject (whose no-args branch produces the full default
  config.json — matches the legacy ensureConfigFile contract).
- gsd-tools.cjs `case 'config-ensure-section'` restored to its
  Phase 6 _dispatchNonFamily form (no CJS fallback — the SDK

configNewProject defaults from canonical manifest:

  back to the padding-tolerant numeric. searchPhaseInContent now does
  two-pass lookup. Without this, `roadmap get-phase PROJ-42` returned
  not-found even when ROADMAP contains `### Phase PROJ-42:`, and
  bare `42` queries cross-matched the PROJ-42 heading.

roadmap-mode-field (1) — roadmapAnalyze surfaces `mode` per phase:
  Extracts the same `**Mode:**` field that roadmapGetPhase already
  parses (CONTEXT.md "MVP Mode" glossary). Without this, downstream
  consumers reading roadmap.analyze output couldn't tell which phases
  were MVP-mode.

bug-3601 (2 subtests) — phase.remove preserves peer-depth decimals:
  Ported the depth-aware end-of-section regex from CJS phase.cjs
  (named capture `(?<h>#{2,4})` + `\k<h>(?!#)` backreference). Now
  removing `### Phase 2:` stops at `### Phase 2.1:` (same depth, peer
  decimal) while continuing past `#### Phase 27.1:` (child depth).

bug-3602 (1 subtest) — phase.remove renumbers slugged plan refs:
  Extended the padded-plan-reference pattern with optional kebab-case
  slug segments `(?:-[A-Za-z][A-Za-z0-9-]*)*` between NN-NN and the
  PLAN/SUMMARY suffix, matching CJS phase.cjs:#3602 fix. Without this,
  `07-01-cherry-pick-foundation-PLAN.md` references stayed at `07-01-`
  after Phase 7 was removed, while the file on disk was already
  `06-01-...`.

config.test (1) — config-get git.base_branch returns "Key not found":
  configNewProject now filters out manifest keys legacy CJS init does
  NOT materialize: top-level `resolve_model_ids`, `context_window`,
  `mode`, `planning`, `graphify`; nested `git.base_branch`. These have
  their own resolution paths (origin/HEAD auto-detect for base_branch,
  feature opt-in for planning/graphify) and materializing the manifest
  defaults would suppress them. Manifest stays the schema source of
  truth per ADR §6; init shape stays minimal per legacy CJS contract.

gsd-sdk-query-registry-integration (1) — agents/gsd-intel-updater.md
references retargeted from `gsd-sdk query intel.*` to `gsd-tools intel
<subcommand>`. intel is out-of-seam per ADR §3 / PRD L160 ("CJS-only
Module handlers ... keep their in-process CJS implementations").
Removing the SDK catalog entries (cf4dd0cb) made the SDK route invalid;
the agent now correctly invokes the CJS handler via gsd-tools, which
routes through Shell Command Projection for cross-platform formatting.

Local: 79/79 across the failing test files. Docker re-run in progress.

   via the CJS router. Closes the

New scripts/lint-docs-required.cjs + Docs Required CI workflow fail any
PR whose changeset fragment is typed Added / Changed / Deprecated / Removed
without modifying at least one file under docs/.

Mirrors scripts/changeset/lint.cjs: pure evaluateLint({ changedFiles,
fragments, labels }) returning { ok, reason, triggering } over a frozen
LINT_REASON enum; CLI wrapper reads the PR diff and parses each touched
changeset fragment via the existing parseFragment helper.

Escape hatches:
- no-docs PR label (global)
- per-fragment <!-- docs-exempt: <reason> --> marker, all triggering
  fragments must carry it for the PR to pass

Fixed and Security fragments do not trigger the lint — bug fixes restore
documented behavior, they do not introduce new behavior to document.

PR templates (enhancement.md, feature.md) gain a Documentation checklist
section pointing at the which-doc-to-update matrix. CONTRIBUTING.md adds
a Documentation Updates section codifying that matrix, the English-canonical
language policy for docs/ and the root README, and the two opt-out routes.

* Address Codex review: fail-closed on malformed fragments and strip docs-exempt marker from rendered release notes (#3213)

Two P2 issues caught by `codex review --base main`:

1) Malformed fragments could silently bypass docs enforcement. parseFragment
   would return ok:false on a triggering Added fragment with bad frontmatter
   and readFragmentsFromDisk dropped it, so evaluateLint saw no triggering
   fragments and passed. The changeset-required lint only checks fragment
   _presence_ not _validity_, so the assumed fallback did not catch it.

   Fix: readFragmentsFromDisk now returns { fragments, malformed }; evaluateLint
   accepts a malformed param and emits a new FAIL_MALFORMED_FRAGMENT verdict
   that outranks every OK path (including the no-docs label) — a parse failure
   must be fixed before docs lint can decide anything else.

2) The per-fragment <!-- docs-exempt: reason --> marker lived in the fragment
   body, so the existing changelog (serializeChangelog) and GitHub release-notes
   (formatBullet) serializers published it verbatim. Worse, both serializers
   append `(#NNNN)` to the body's last line — with the marker as the trailing
   line, the PR suffix attached to the hidden comment instead of the visible
   bullet.

   Fix: parseFragment now extracts the marker into a typed `docsExempt` field
   and strips it from `body`, so all downstream renderers produce clean output
   without remembering to strip. The regex is anchored to its own line (^...$
   with m flag) so inline mentions of the marker syntax in documentation
   (e.g. inside backticks) cannot accidentally exempt a fragment. Bounded
   character class [^\n>] keeps the regex linear-time.

Test additions:
- tests/lint-docs-required.test.cjs: FAIL_MALFORMED_FRAGMENT coverage,
  end-to-end "Added fragment with bad pr → malformed → fail-closed" regression
  test, updated readFragmentsFromDisk return-shape assertions, isExemptFragment
  now checks the typed docsExempt field rather than body content.
- tests/changeset-parse.test.cjs: extractDocsExempt extraction cases (with/
  without reason, case-insensitive, EMPTY_BODY when body is only a marker),
  inline-mention false-positive guard, real-marker-wins-when-also-inline test.
- tests/changeset-new.test.cjs: fragment shape now includes docsExempt: null.

CONTRIBUTING.md updated to clarify the "on its own line" requirement and the
parse-time stripping behavior. The bootstrap fragment cleaned up so its body
no longer contains a literal marker example that would have triggered the
false-positive case.

Full suite: 9696/9696 pass.

* CRLF-safe docs-exempt marker stripping (Codex review pass 2, #3213)

Second `codex review --commit` pass caught a CRLF regression in the
docs-exempt extraction added in the previous commit.

Repro: a Windows-authored fragment

  ---\r\ntype: Added\r\npr: 1\r\n---\r\nFeature.\r\n\r\n<!-- docs-exempt: x -->\r\n

would parse to body `Feature.\r\n\r\n\r` because:

  - The previous trailing-newline slice trimmed only `\n`, leaving `\r`.
  - DOCS_EXEMPT_RE was anchored with `$` only — in multiline mode `$`
    matches before `\n` but does not consume `\r`, so the marker line's
    trailing `\r` was left behind after replace.
  - The cleanup regex stripped trailing `\n` but not `\r`.

Net effect: serializeChangelog emitted

  - Feature.\r
  \r
  \r (#1)

— the `(#1)` PR suffix landed on a blank line instead of attached to
the visible bullet. Same bug surfaces in github-release-notes formatBullet.

Fix:
- DOCS_EXEMPT_RE: add `\r?` before `$` so the regex consumes the CR of a
  CRLF terminator. Switch reason character class from `[^\n>]` to
  `[^\r\n>]` so CRLF-authored reasons don't carry a trailing `\r`.
- extractDocsExempt cleanup: `[ \t\r]+$/gm` strips trailing `\r` on each
  line; `(?:\r?\n){3,}` collapses CRLF triple-blank-lines; `[\r\n]+$`
  strips every trailing line terminator (LF or CR).
- parseFragment trailing-newline slice: CRLF-aware — strips `\r\n` (2
  chars) before falling through to single `\n`.

Tests: two CRLF regression cases in tests/changeset-parse.test.cjs —
Codex's exact repro (end-to-end through serializeChangelog) plus the
no-marker CRLF passthrough case. Full suite: 9698/9698 pass.

* CRLF regression test asserts on parseChangelog IR not rendered text (Codex review pass 3, #3213)

Third `codex review` pass caught that the CRLF regression test added in
the previous commit asserted on serializeChangelog's rendered Markdown
via `out.split('\n')` + `assert.match`. That violates CONTRIBUTING.md's
"Prohibited: Raw Text Matching on Test Outputs" rule and the documented
serializer contract in `serialize.cjs`:

  > tests assert via round-trip (parse(serialize(ir)))
  > rather than by inspecting serialized text

Replace the regex check with the established `parseChangelog(out)`
round-trip and assert on the structured `{ body: 'Feature.', pr: 1 }`
bullet. This is also a stronger regression check than the substring
match: Codex's own probe in the review session confirmed the pre-fix
buggy body shape (`Feature.\r\n\r\n\r`) breaks parseChangelog's bullet
regex entirely (returns `bullets: []`), so the round-trip catches the
exact failure mode end-to-end.

Full suite: 9698/9698 pass.

* Address CodeRabbit findings: anchor link + require non-empty docs-exempt reason (#3213)

CodeRabbit's review on the PR caught two actionable issues, both quick wins.

Anchor link in PR templates pointed to a heading that does not exist. The
CONTRIBUTING.md heading "Documentation Updates — Update the Relevant Docs"
contains an em-dash, which GitHub strips entirely when generating anchor
slugs (it does NOT collapse to a hyphen). The actual anchor is
#documentation-updates-update-the-relevant-docs (single hyphen between every
word), not #documentation-updates--update-the-relevant-docs (double hyphen
where the em-dash was). Both feature.md and enhancement.md fixed.

The docs-exempt marker matched a bare `<!-- docs-exempt -->` with no reason,
which defeats the entire purpose of the escape hatch — the marker exists to
leave an audit trail explaining WHY a PR is exempt. Without a reason it is
a silent bypass.

Fix: DOCS_EXEMPT_RE now requires both the colon AND a non-whitespace first
reason character. Bare `<!-- docs-exempt -->`, empty `<!-- docs-exempt: -->`,
and whitespace-only `<!-- docs-exempt:   -->` are all rejected as if the
marker were not present (`docsExempt: null`). The lint then falls through
to its normal docs-required / no-docs-label checks.

`isExemptFragment` in the lint module tightened too — defense-in-depth: even
if a caller constructs a fragment with `docsExempt: ''` directly, it does
not count as exempt. The predicate now requires `typeof === 'string'` and
non-empty after trim.

Tests:
- changeset-parse.test.cjs: three new explicit-rejection cases (bare marker,
  empty reason, whitespace-only reason). Existing DOCS_EXEMPT_RE shape test
  extended with negative assertions for the same three forms.
- lint-docs-required.test.cjs: prior "empty reason still exempt" test
  inverted — empty/whitespace docsExempt now produces FAIL_DOCS_MISSING.
  isExemptFragment helper test extended with the same negative cases.
- CONTRIBUTING.md: clarified that the reason is required and non-empty.

Skipped CodeRabbit's third finding ("use `npm run lint:docs` in CI workflow
instead of `node scripts/lint-docs-required.cjs`") — the existing
changeset-required.yml uses the direct-node form for the equivalent
changeset lint, so the new docs-required.yml is convention-consistent.
Switching one without the other would create drift, and switching both is
out of scope for #3213.

Bootstrap fragment continues to extract cleanly under the stricter regex
(verified — `docsExempt` field still contains the full bootstrap reason).
Full suite: 9701/9701 pass.

e82876fe45 feat(3597): split test suites and add Node 22/24/26 OS matrix
scripts/run-tests.cjs gains `--suite <name>` filtering using a filename
suffix convention (`*.security.test.cjs`, `*.integration.test.cjs`, …).
Files with no marker are `unit` (the default fast lane); files with a
marker land in the matching suite. No `--suite` flag preserves the prior
behavior of running every test (backcompat for `npm test` and
`npm run test:coverage`).

New package scripts wire the suites to stable entrypoints:
test:unit, test:integration, test:install, test:security, test:slow,
test:coverage:unit, test:coverage:all. Unknown suite → exit 2 with the
list of valid suites; empty suite → exit 0 with a stderr notice so empty
lanes (e.g. `security` before adversarial tests land) don't gate CI.

CI matrix grows from `ubuntu × {22,24}` + a single macOS lane to
`{ubuntu, macos, windows} × {22, 24, 26}`. `fail-fast: false` so one
lane failure doesn't cancel siblings. Node 26 is `continue-on-error`
until actions/setup-node stabilises that image. PR CI runs unit +
integration + security on every cell; `install` and `slow` only on
`main` push. A dedicated `coverage` job runs `test:coverage:unit` on
ubuntu/Node 24 and uploads the report.

Grouping policy lives in docs/TESTING-SUITES.md with a pointer from
CONTRIBUTING.md. New harness test covers arg parsing, filter selection,
empty-suite behavior, and failure propagation.

66a429adf4 docs(3621): forbid bundling test-fixture updates into docs: commits (#3625)
Adds a Pull Request Guidelines bullet making explicit what v1.42.3
hotfix taught us the hard way: when a production change makes an
existing test assertion stale, the test correction must be its own
test: (or fix:) commit, not bundled into a docs: commit that also
explains the change.

The release-sdk hotfix cherry-pick filter routes by commit-subject
prefix (fix:, chore:, test: — see release-sdk.yml). A docs: commit
that hides a test fix is invisible to the picker. The result is a
half-shipped state on the hotfix branch: production code changed,
test assertion stale, CI red.

This is the upstream contributor-side mitigation. The picker-side
fix landed in PR #3623 (broadens the prefix filter and the
classifier's CI-gating-path detection); this PR documents the
upstream discipline that keeps the picker from getting fooled in
the first place.

The repo's sequential ADR/PRD numbering convention has produced
recurring collisions when developers compute "next number" locally
and ship in parallel — currently visible on disk as duplicate
docs/adr/0010-*.md and triplicate docs/adr/0011-*.md, plus a stack
of "resolve ADR conflict" commits in git history.

Replace the local-compute convention with issue#-prefix slug naming:

  docs/adr/<issue#>-<slug>.md     (new ADRs)
  docs/prd/<issue#>-<slug>.md     (new PRDs — directory introduced)

GitHub issue numbers are server-assigned and atomic, so the
reservation step the CONTRIBUTING.md issue-first rule already enforces
also produces the artifact ID. One issue = one ADR-or-PRD = one PR.
Same shape as the existing changeset random-name pattern (#2975) for
CHANGELOG.md fragments, applied to a different artifact class.

Migration policy: legacy ADRs 0001-* through 0011-* are preserved
as immutable historical record. The new convention applies only to
ADRs/PRDs created on or after this merge.

Files updated:
- docs/adr/README.md        — naming convention + legacy note + link
- docs/prd/README.md (new)  — seeds the new directory + same convention
- CONTRIBUTING.md           — new "Proposing an ADR or PRD" section
- docs/contributor-standards.md — formalize as contributor requirement

No code surface — docs-only.

Recently bit on #2960/#2972 in the same session — fix-the-conflict-and-rebase
tax. Replace the shared-file model with per-PR fragment files that never
share lines.

Implementation built TDD per #2975, vertical slices with structured-IR
assertions throughout:

  - Preserve fragment body verbatim. Previously body.trim() ate
    intentional leading whitespace (code blocks, etc.); now trim() is
    used only for the emptiness check, and a single trailing newline
    is stripped (the editor-added one) so well-formed fragments
    round-trip byte-for-byte. Added a regression test asserting a
    code-block-leading body is preserved.

scripts/changeset/cli.cjs
  - Validate flag values during argument parsing. parseArgs now returns
    { ok, opts | error }; rejects `--repo` etc. with no following value
    or with another flag as the value. main() surfaces the error
    message before exiting 2.
  - Handle post-write fragment-deletion failures. After CHANGELOG.md
    is written, any unlink failure is captured into a structured
    deleteFailures list with reason 'fail_fragment_delete'; cmdRender
    returns exitCode=1 with the partial-failure detail instead of
    leaving the changelog updated and fragments behind (which would
    cause double-consumption on rerun).

scripts/changeset/lint.cjs
  - Treat CHANGELOG.md as a linted user-facing path. Direct edits to
    CHANGELOG.md (the bypass route around the new workflow) now fail
    the lint with FAIL_MISSING_FRAGMENT. Added a regression test for
    that case.
  - Use cp.execFileSync instead of cp.execSync for the git diff call.
    Eliminates the shell-interpolation surface on GITHUB_BASE_REF;
    git's own arg parser remains the validator.

scripts/changeset/new.cjs
  - Atomic fragment creation. existsSync() + writeFileSync was racy
    under concurrent invocations. Now writeFileSync uses { flag: 'wx' }
    which fails EEXIST on collision; the random-name retry loop
    catches EEXIST and re-rolls. Throws explicitly after 16 attempts
    rather than silently overwriting.

.changeset/README.md
  - Add language tag `md` to the format example fence (markdownlint
    MD040).

All 25 changeset tests pass; lint clean (356 test files, 0 violations).

* fix(#2975): sanitize --type and validate flag values in new.cjs (CR fixes)

Two CR findings on scripts/changeset/new.cjs:

1. (Minor) `type` was embedded in frontmatter without sanitization. A
   newline in the value (e.g. `--type 'Fixed\ntype: Added'`) would
   corrupt the fragment. scaffoldFragment now validates `type` against
   the Keep-a-Changelog ALLOWED_TYPES set BEFORE writing — same set
   parse.cjs uses on consume. Throws with a typed error referencing
   the allowed values; tests cover the newline case + 4 other
   non-allowed values.

2. (Minor) `--repo` (and other value-taking flags) without a value
   silently set opts.repo to undefined, which produced a cryptic
   ERR_INVALID_ARG_TYPE deep inside path.join. parseArgs now mirrors
   the cli.cjs convention: returns { ok, opts | error }, validates
   that the next token exists and is not itself another flag, and
   surfaces a precise "missing value for --repo" message before exit.
   Added 3 tests: missing-trailing-value, flag-as-value, well-formed.

29 tests pass across the changeset suite (4 new regression tests).

ef43f5161f fix(#2969): deterministic Step 5 verification gate for /gsd-reapply-patches (#2972)
* fix(#2969): deterministic Step 5 verification gate for /gsd-reapply-patches

The prior Step 5 "Hunk Verification Gate" was prescribed correctly in the
workflow text — but executed laxly by the LLM, which filled in `verified: yes`
without actually checking content presence. The reporter observed three
distinct files (skills/gsd-discuss-phase/SKILL.md, skills/gsd-autonomous/
SKILL.md, get-shit-done/workflows/new-project.md) where archives contained
substantive user-added blocks that did not survive into the merged result, yet
the gate reported clean.

Move verification from LLM-driven prose into a deterministic Node script the
workflow calls. The script can't be shortcut.

Changes:

- scripts/verify-reapply-patches.cjs (new): pure Node, no external deps.
  For each file in the patches dir, computes user-added significant lines as
  the line-set diff between backup and pristine baseline (when available;
  falls back to "every significant backup line" when no pristine — over-broad
  but the safe direction for this bug class). Asserts each line appears
  literally in the merged installed file via String.prototype.includes.
  Filters trivial lines (length < 12 chars, pure punctuation, decorative
  comments) so harmless drift doesn't trigger false failures. Exits 0 on
  pass, 1 on any miss with per-file diagnostic, 2 on usage error.
  Supports --json for workflow consumption.

- get-shit-done/workflows/reapply-patches.md: rewrite Step 5 to call the
  script and parse its JSON output. The Step 4 Hunk Verification Table
  remains as advisory Claude-readable summary, but the gate is now the
  script's exit code.

- tests/bug-2969-verify-reapply-patches.test.cjs (new): 6 tests covering
  (a) pass when every line survives, (b) fail when a line is missing,
  (c) fail when the merged file is deleted entirely, (d) --json structured
  report shape, (e) backup-meta.json is correctly skipped as metadata,
  (f) no-pristine-dir fallback exercises the safe over-broad path. All pass.

Out of scope: the manifest-baseline tightening described in #2969 Failure 1
(saveLocalPatches comparing against the wrong baseline so prior silent wipes
poison subsequent updates). That's a separate, bigger architectural change
involving pristine-content infrastructure; this PR addresses the gate fidelity
half so users at least see the diagnostic when content goes missing.

Closes #2969 (partial — Failure 2 only)

* fix(#2969): preserve #1999 Hunk Verification Table assertions alongside new script gate

CI failure on PR #2972 surfaced that tests/reapply-patches.test.cjs (the
#1999 contract) asserts Step 5 references:
  - "Hunk Verification Table"
  - `verified: no` failure condition
  - explicit STOP/halt/abort directive
  - "table absent / missing" halt path

My initial Step 5 rewrite for #2969 substituted the deterministic script
for the table-based gate entirely, stripping those references. The script
is the strictly stronger gate, but the existing #1999 test enforces the
table-based safety net as a defense-in-depth contract.

Restore both gates as a layered Step 5:

  - 5a (binding): deterministic verifier script — script gate, exits
    non-zero on any miss, cannot be shortcut by the LLM
  - 5b (advisory): Hunk Verification Table review — preserved as
    redundant safety net for the case where the script has a bug or the
    pristine baseline is unavailable

Both gates must pass. Verified: tests/reapply-patches.test.cjs (5 tests
in the #1999 suite) and tests/bug-2969-verify-reapply-patches.test.cjs
(6 tests in the #2969 suite) all pass — 21/21 total in this fixture.

* fix(#2969): address CodeRabbit findings on workflow + script

Five CR findings on PR #2972, all valid; addressed in this commit:

1. (Major) Stderr was merged into VERIFY_OUTPUT via `2>&1`, so any Node
   warning, deprecation notice, or stack trace would corrupt the JSON
   parse downstream. Capture stdout only; stderr remains on the
   controlling terminal for operator visibility.

2. (Major) verifyFile() crashed with EISDIR/EACCES instead of producing
   a structured diagnostic when the installed path was a directory or
   unreadable. Wrap statSync/readFileSync in try/catch and emit a
   per-file fail row; the whole-run gate continues with structured
   output. Added test case asserting the directory-at-installed-path
   case fails with `not a regular file` diagnostic instead of crashing.

3. (Minor) PRISTINE_FLAG built as a single string + unquoted expansion
   would split paths with spaces. Switched to a bash array (VERIFY_ARGS)
   that preserves whitespace through expansion.

4. (Minor) Fenced code block missing language tag (markdownlint MD040).
   Added `text` tag to the error message block.

5. (Minor) Usage comment said pristine fallback was "backup-meta lookup"
   but the actual code path falls back to significant-line checks from
   backup content. Corrected the comment to match implementation.

Verified all 21 tests in tests/reapply-patches.test.cjs (#1999 contract)
+ tests/bug-2969-verify-reapply-patches.test.cjs (now 7 tests with the
new directory case) pass.

* test(#2969): structured JSON assertions, no substring matching on script output

Replace every assert.match(r.stdout, /pattern/) call with structured
assertions on the parsed JSON report from the script's own --json mode.
The script's --json contract IS the structured shape we test against —
the test author should never depend on the human-readable formatter
output, just as no test should depend on substring presence in source.

Changes:

  - All 7 tests now run the verifier with --json (via a runVerifier()
    helper) and parse the resulting JSON document into { status, report,
    stderr }. Diagnostic stderr is preserved as a separate channel for
    debug output but is not used for assertions.
  - Each previously substring-matched diagnostic ("Failures: 1",
    "not a regular file", "installed file missing after merge",
    file path, dropped line) is now a deepEqual / equal / Array.includes
    against typed report fields: report.failures, report.results[i].status,
    report.results[i].reason, report.results[i].file,
    report.results[i].missing[].
  - Added an explicit "documented shape" test asserting the JSON output
    has exactly the keys { file, missing, reason, status } per result —
    locks the public contract of the --json mode.
  - DRY'd up fixture reset into a resetFixture() helper since every test
    starts with a fresh patches/installed/pristine triple.

Linter: scripts/lint-no-source-grep.cjs reports 0 violations across 348
test files. Combined run of bug-2969-...test.cjs (7 tests) +
reapply-patches.test.cjs (5 tests in the #1999 suite) all pass —
22/22 in the relevant fixture.

* fix(#2969): typed REASON enum + raw-text-matching rule shipped repo-wide

This commit closes the loop on the no-source-grep discipline:

1. scripts/verify-reapply-patches.cjs:
   - Frozen REASON enum exposes the diagnostic surface as stable codes:
     OK_NO_USER_LINES_VS_PRISTINE, OK_NO_SIGNIFICANT_BACKUP_LINES,
     FAIL_INSTALLED_MISSING, FAIL_INSTALLED_NOT_REGULAR_FILE,
     FAIL_READ_ERROR, FAIL_USER_LINES_MISSING.
   - Each result.reason is now a code from this enum, not free text.
     Tests assert via REASON.X equality, not regex on prose.
   - REASON exported from module.exports.

2. tests/bug-2969-verify-reapply-patches.test.cjs:
   - Full rewrite. Every assertion on typed structured fields:
     report.results[0].status === 'fail',
     report.results[0].reason === REASON.FAIL_INSTALLED_NOT_REGULAR_FILE,
     report.results[0].missing.includes(droppedLine) (Array set membership,
     not String substring).
   - Locks the REASON enum surface via Object.keys(REASON).sort() deepEqual.
   - Locks the JSON report shape via Object.keys(report).sort() deepEqual.
   - Zero regex, zero String#includes, zero startsWith/endsWith on text.

3. CONTRIBUTING.md:
   - New section "Prohibited: Raw Text Matching on Test Outputs" with
     concrete BAD/GOOD examples (substring on file content; assert.match
     on stdout; "structured parser" hiding string ops; regex on free-form
     reason fields).
   - The rule statement: "Tests assert on typed structured values. If
     the code under test produces text, the code under test must also
     expose a structured intermediate representation, and the test must
     assert on that IR — never on the rendered text."
   - Required structured-surface table: file IR, --json mode, frozen
     enum, fs facts.
   - "Hiding grep behind a function is still grep" callout — the
     parser-wrapper anti-pattern.
   - New `pre-existing-text-matching` exemption category for the 8
     grandfathered files. Marked Transitional; new tests cannot use it.

4. scripts/lint-no-source-grep.cjs:
   - Three new patterns enforced (in addition to the existing .cjs-source
     readFileSync rule):
     - assert.match/doesNotMatch on .stdout/.stderr
     - .stdout/.stderr.<includes|startsWith|endsWith>(
     - readFileSync(...).<includes|startsWith|endsWith>(
   - Aggregated violations per file (multiple findings now report together).
   - Updated diagnostic message references both CONTRIBUTING.md sections.

5. 8 pre-existing tests annotated with `// allow-test-rule:
   pre-existing-text-matching` so the lint passes on this commit; each
   carries the prose "Tracked for migration to typed-IR assertions; do
   not copy this pattern." Files: bug-2649, bug-2687, bug-2796, bug-2838,
   bug-2943, graphify, hooks-opt-in, security-scan.

Verification: lint 0 violations across 348 test files; full suite passes.

* fix(#2969): rename exemption category to pending-migration-to-typed-ir + cite tracking issue

Per maintainer feedback:
1. "Grandfathered" / "legacy" framing is wrong — both terms imply
   permanent or condoned exemption. The 8 files are tracked for
   correction, not exempted.
2. Each annotated file must cite the tracking issue so the migration
   work is auditable.

Changes:
- CONTRIBUTING.md: rename exemption category from
  `pre-existing-text-matching` to `pending-migration-to-typed-ir`. Update
  prose to "Tracked for correction, not exempted" and require each
  annotation to cite the open migration issue (e.g.
  `// allow-test-rule: pending-migration-to-typed-ir [#NNNN]`).
- 8 test files: update annotation to cite #2974 (the tracking issue
  opened for migrating these files to typed-IR assertions).

- Adds allow-test-rule annotations with explanatory comments to 7 legitimate
  structural tests: architectural invariants (locking, orphan-worktree),
  structural regression guards (milestone-regex-global), docs-parity
  (config-field-docs), integration-test-input (copilot-install), and
  structural-implementation-guards (bug-1891, discuss-mode)

Updates CONTRIBUTING.md Testing Standards section with:
- "Prohibited: Source-Grep Tests" section with the before/after pattern,
