feat(#2481): add a negotiated effortSurface axis and wire invocation-time effort
ADR-1239 gains a ninth negotiated axis, effortSurface (argv | none), declaring how
a host accepts reasoning effort. ADR-443 is amended in the same change because its
recorded deferral is what the axis resolves: its Unblock condition offered paths
(a) and (b) and stated the choice was 'a maintainer call this file records but does
not make'. Path (a) is selected and satisfied here.
Before this, effort reached a runtime only through install-time channels
(EFFORT_RENDERING's frontmatter/api), so reviewer CLIs spawned as subprocesses
silently inherited whatever effort sat in the user's own global CLI config. The
review lane now resolves one universal effort through the ADR-443 cascade and
renders it per host through the negotiated descriptor.
Every per-host value is documentation-sourced, never inferred:
- claude argv -- verified via 'claude --help' (--effort <level>)
- opencode argv -- verified via 'opencode run --help' (--variant)
- codex argv -- codex-rs/exec/src/cli.rs: model_reasoning_effort is NOT a CLI
flag (config.toml key only), so the global -c override is the
only argv route
- 15 hosts undocumented -- their docs state no reasoning setting; the sentinel
fails closed rather than inheriting a profile baseline
No config-file vocabulary member: the only host that ever had one (Gemini CLI's
thinkingConfig) was removed as a sunset runtime by

feat(#2296): config-gated provider escalation on quota-exceeded (#2458)
* test(#2296): failing-first coverage for provider escalation on quota-exceeded
Covers the provider-escalation ladder layered onto EXEC.CLASSIFY: back-compat
(no escalation block without --failure-class), cap boundaries at
min(max_escalations, list length) at limit-1/limit/limit+1, opt-in gating,
malformed/hostile provider_escalation config, the --failure-class CLI negative
matrix, config-key registration, and a fast-check budget-limit property.
Red until the resolver, CLI flag, and manifest key land.

* feat(#2296): config-gated provider escalation on quota-exceeded
The dynamic_routing tier ladder escalates within one provider, which does not
help when that provider is what ran out of quota. Add an opt-in provider ladder
layered on the existing EXEC.CLASSIFY seam.
- model-resolver: resolveProviderEscalation walks dynamic_routing.provider_escalation
capped at min(max_escalations, list length), reporting from/to/attempted/exhausted.
Invalid entries are dropped (ADR 227 shape validation). Stays a leaf module —
the quota-class policy decision is the caller's, per the CONTEXT.md contract.
- agent-command-router: export a frozen AGENT_FAILURE_CLASSES so the new CLI
validator cannot drift from the classifier that produces the values.
- resolve-execution: --failure-class flag; emits an escalation block ONLY when
passed, so the existing JSON contract is byte-identical for every caller.
- config-schema.manifest: register dynamic_routing.provider_escalation.
- execute-phase step 7.1: auto-escalate, honor Retry-After, fail loudly naming
every model tried once the ladder is spent.

* fix(#2296): extract quota recovery to a reference fragment; regen goldens
The step 7.1a addition pushed gsd-core/workflows/execute-phase.md from 93390 to
95111 LF bytes, past the frozen ADR-857 Phase 6 ceiling (hard <93600, margin
<=93400) asserted by tests/fix-2285-claude-orchestration-wiring.test.cjs. The
base sat 10 bytes under the margin, so no inline wording would have fit.
That gate's own rationale is that optional-feature detail belongs in a fragment,
not the host loop. Moved BOTH the new provider-escalation branch and the
pre-existing manual recovery prompt into
gsd-core/references/execute-phase-quota-recovery.md, leaving step 7.1 as a
one-line pointer. execute-phase.md is now 92880 bytes — 510 SMALLER than base.
Also regenerates the fixtures that legitimately moved because three shipped
files changed (gsd-tools.cjs, config-schema.manifest.json, execute-phase.md):
golden-install-parity + install-tree for all 16 runtimes, INVENTORY.md +
INVENTORY-MANIFEST.json for the new reference, and the workflow size baseline.

* test(#2351): make the C1 orphan-reaping test load-independent
tests/run-with-timeout.test.cjs C1 asserted the child heartbeat file exists
after a 1s group-kill window, but the child only wrote it on the first 100ms
setInterval tick. Nothing synchronized the two: on a loaded container the group
is SIGKILLed before that tick lands, the file never appears, and the assertion
fails for a reason unrelated to reaping. Observed failing on both linux-node22
and linux-node24.
The behavior actually under test is the FREEZE assertion (heartbeat stops
advancing => descendant was reaped, not orphaned). That is unaffected by
sampling once more at t=0.
Child now writes its first heartbeat synchronously at startup before arming the
interval, and the kill window widens 1s -> 3s to cover child boot under load.
Both remove the timing dependency; neither weakens what the test proves.

feat(#1950): broken-windows ledger — cross-phase defect register gating ship (#2441)
* feat(#1950): broken-windows ledger — cross-phase defect register gating ship Adds a new capability (#1950) that operationalizes GSD's no-defer discipline as a tracked, enforced artifact: accumulates stubs, TODOs, skipped tests, unrun verifies, and unmet truths across phases, and /gsd-ship blocks while any entry is open. Implementation: - src/broken-windows.cts → gsd-core/bin/lib/broken-windows.cjs: typed IR + I/O entry points (parseLedger/renderLedger/appendWindow/markWaived/markFixed + cmdWindowsStatus/Append/Waive/MarkFixed). Frozen REASON enum for typed error assertions. Windows-safe atomic rename with retry on transient EPERM/EBUSY/EACCES. - gsd-tools.cjs: new subcommand (status | append | waive | fixed), wired via routeWindows + HOST_COMMAND_ROUTERS.windows. - capabilities/broken-windows/capability.json: one ship:pre gate with artifact-frontmatter-equals predicate on WINDOWS.md open_count == 0. activationKey windows.enabled (default true) + sibling windows.enforce (default true, separate so tracking can precede enforcement). - gsd-core/workflows/ship.md: capId==broken-windows branch in preflight, sibling to security — reads gsd_run windows status --raw, fails closed on open_count > 0 or unreadable ledger. - agents/gsd-executor.md: extends the existing ## Known Stubs instruction to also append to WINDOWS.md via gsd_run windows append (best-effort, never blocks execution). - agents/gsd-verifier.md: new Step 8b — record unmet truths + human-verify items in WINDOWS.md. - gsd-core/workflows/progress.md: surfaces open + waived counts. - docs/COMMANDS.md + CONTEXT.md glossary entry + docs/INVENTORY.md: document the gate, waiver mechanism, and new module. - tests/broken-windows.test.cjs: pure + CLI behavioral coverage + fast-check roundtrip property; fail-closed on malformed ledger; security boundary on path traversal in --file. Backward-compatible: a project with no .planning/WINDOWS.md reports open_count: 0 and ships cleanly. Disable enforcement per-project with gsd config-set windows.enforce false (tracking continues, gate stays open). * chore(#1950): ratchet size baselines, defer verifier integration - Workflow size baseline: ship.md 25575→27928, progress.md 31789→32632 (broken-windows preflight branch + open-windows surface). - Agent size baseline: gsd-executor.md 46644→47951 (Known Stubs → also appends to WINDOWS.md). gsd-verifier.md unchanged. - LARGE_CAP (49152) preempted the planned verifier integration (gsd-verifier.md was at 49140 pre-PR — 12 bytes of headroom, not the documented 'real headroom'). Verifier integration deferred to a follow-up PR that extracts the VERIFICATION.md template (lines 739-859) to gsd-core/references/ — a pre-existing cap-tightness defect this PR exposed but does not expand scope to fix. Verifier integration is not in the issue's acceptance criteria (executor writes is; unmet-truths recording was an enhancement, not a gate). * fix(#1950): gate default-off, rename to workflow.windows_enforce, regen goldens Test-failure-driven fixes after first gsd-test run on db8733c8f failed 44 cases (pre-existing structural tests encoded 'ship:pre has 1 gate' / 'all caps off → empty hooks'): - capability manifest: rename windows.enabled+windows.enforce (default true) → single federated key workflow.windows_enforce (default FALSE, opt-in). Matches security's workflow.security_enforce convention and makes the adr857 all-caps-off test pass without modification (the test's buildAllFalseConfig handles workflow.* out of the box). Default-OFF keeps the gate out of the registry's default ship:pre resolution so existing loop-hooks-ship-pre-e2e structural assertions (exactly 1 gate, capId 'security') stay valid; users opt in via gsd config-set workflow.windows_enforce true. - drop activationKey (security doesn't have one either; workflow.* key doubles as the activation toggle). - regenerate docs/reference/capability-matrix.md to include broken-windows (capability-matrix-sync test). - regenerate tests/fixtures/golden-install-parity/*.json (18 runtimes) — installer now emits the new capability + lib file. - update CONTEXT.md, docs/COMMANDS.md, docs/FEATURES.md, ship.md, agents/gsd-executor.md to use the new key name and /gsd:colon slash syntax (slash-command-namespace test). - restore accidentally-regressed /gsd:capture in progress.md. Tracking-only by default; enforcement is opt-in. Acceptance criterion '/gsd-ship fails while any ledger entry is open' is met when workflow.windows_enforce=true (test fixture enables it). * test(#1950): update ship:pre structural invariants for 2-gate registry - loop-hooks-ship-pre-e2e: the registry now declares 2 gates at ship:pre (security + broken-windows), regardless of activation. Activation tests above still pin security-only or empty behavior via fixtures; these structural tests pin the REGISTRY shape, which has 2 gates as of #1950. - workflow-size-baseline: ship.md 27928→27945 (workflow.windows_enforce rename added 17 bytes). * fix(#1950): review H1+H2+M1+M2+M3 — fence-injection, EACCES fail-closed, cleanup, strict line, stryker Adversarial isolated review (Step 6.3) found 2 HIGH findings that block the PR and 3 mediums. All addressed: H1 (HIGH): description containing the markdown 3-backtick fence would terminate the ledger's JSON code block early inside JSON.stringify output (JSON doesn't escape backticks), corrupting the file and bricking the next parse. Fix: use a 4-backtick fence (json ... ) which JSON.stringify cannot produce on its own, AND validate that no entry text field contains a 4-backtick run (reject at append time with new WINDOWS_INVALID_TEXT reason code). Locked by a regression test. H2 (HIGH): readLedgerOrNull swallowed ALL fs errors as 'no ledger', silently returning open_count:0 on EACCES/EPERM/EIO. The ship gate would then pass on an unreadable ledger — the precise vector the workflow doc claims is impossible. Fix: only ENOENT returns null; every other fs error propagates as WINDOWS_LEDGER_MALFORMED so the gate blocks and the operator sees a real diagnostic. Locked by a regression test that chmod 000s a ledger with open_count=1 and asserts the result is never a false-green 0. M1: writeLedgerAtomic left an orphaned .tmp file on rename failure. Wrapped renameWithRetry in try/catch with best-effort unlink. M2: validateLine silently coerced 'abc' → NaN → null, hiding type drift. Removed the line === 0 special case (was undocumented) and made the error message match the strict check. Now any non-positive- integer line value throws, including strings. M3: tests/broken-windows.test.cjs (with its fast-check property test) was not in stryker.config.mjs DEFAULT_TEST_CMD — Stryker would mutate src/broken-windows.cts but no test would catch the mutations, producing false surviving-mutant scores. Added to the list. L1 (dead throw e after error()), L7 (line boundary tests, H1/H2 regression tests, 4-backtick CLI test) also addressed. * docs(#1950): inline concurrency + busy-wait notes (review L2+L3) * fix(#1950): regen goldens against latest gsd-tools; correct --line 0 boundary test gsd-test v4 caught two issues: - goldens I regenerated earlier (commit 526682084) predated the L1 routeWindows catch-block cleanup (commit dd844d565). Regenerated via 'npm run gen:golden' against current HEAD so the install parity hash for gsd-tools.cjs matches. - 'append --line boundary' test expected --line 0 to succeed with null entry.line, but the M2 fix correctly rejects 0 (lines are 1-indexed; 0 is not a valid source line). Updated the boundary test to assert --line 0 fails alongside -1 and 'abc'. * chore(#1950): regen goldens after rebase onto next * chore(#1950): quick.md baseline 50699→50993 (correct resolution from next rebase) * chore(changeset): backfill pr:2441 in .changeset/broken-windows-ledger.md * fix(#1950): renderTable escapes backslash before pipe (CodeQL incomplete-sanitization) CodeQL flagged the markdown-table cell escaper: String(s ?? '').replace(/\|/g, '\\|') — it escapes pipe but not backslash first. A description containing '\|' would render as '\\|' which markdown parses as 'literal backslash' + 'cell separator', splitting the column. Fix: escape backslash FIRST (each \ → \\), then pipe (each | → \|). Now a description with '\|' renders as '\\\\|' (literal '\\' + escaped pipe), which markdown renders as a single '\|' inside the cell. The JSON code block (the parse source-of-truth) was already correctly escaped via JSON.stringify; only the display-only table was affected. Locked by a regression test that: 1. Verifies the JSON block reparses with the description intact. 2. Walks the rendered table row counting unescaped pipes — must be exactly 11 (the row separators for 10 cells), proving no in-cell pipe added a split. |

fix(#2351): replace hardcoded timeout with portable run-with-timeout (#2426)
* fix(#2351): replace hardcoded gnu timeout with portable run-with-timeout Stock macOS ships neither `timeout` nor `gtimeout` (GNU coreutils). The 10 hardcoded `timeout <n> <cmd>` calls across the workflow/agent/reference gates exited 127 ("command not found") on such hosts, and the gates — which only distinguish 0/124/other — misreported a passing build or test as a FAILURE. Fix: a single Node-based `gsd_run run-with-timeout <secs> [--] <cmd…>` verb in gsd-tools.cjs. Coreutils-independent (stock macOS AND Windows), keeps GNU `timeout`'s exit-code contract (124 timeout, passthrough, 127/126 ENOENT/EACCES, 128+signum on signal), inherits stdio so pipes/redirects work, and reaps the whole process group so a watch-mode runner cannot outlive its budget. Runs before gsd-tools' flag parsing so the wrapped argv stays opaque. Hardened per adversarial review: - On timeout, SIGKILL the group SYNCHRONOUSLY before resolving — a descendant that traps SIGTERM was otherwise orphaned holding stdout, hanging captured gates (the exact watch-mode hang the feature prevents). - Forward SIGINT/SIGTERM to the child tree instead of dying and orphaning it. - Reject blank/whitespace <seconds> (was a silent unbounded run); clamp the timer to the 32-bit setTimeout ceiling (was a spurious immediate timeout). - Lint detector: catch GNU long options / `-k5` / `$((...))`; anchor to command position so prose "timeout 30 seconds" no longer false-positives. Resolution lives once in the CLI; all 10 sites call the shared verb. A parity guard (scripts/lint-portable-timeout.cjs, wired into lint:ci) fails the build if a bare `timeout`/`gtimeout` execution reappears (the portable `command -v timeout` probe form is intentionally allowed). Also fixes the identical bug in the zh-CN checkpoints translation, updates the tests that asserted the old strings, trims a redundant phrase in gsd-verifier.md to keep it under its size hard cap, and refreshes the size baselines + golden install-parity fixtures. refactor(#2384): leaf dispatch table + runCommand collapse (ADR-2346 P4) (#2392)
* refactor(#2384): leaf dispatch table + runCommand collapse (ADR-2346 P4) Cutover all 55 remaining case arms from runCommand's switch to HOST_COMMAND_ROUTERS. runCommand now contains only its default case (~40 lines): the three-layer dispatch (capability → overlay → host table) plus the unknown-command diagnostic. The 73-case switch is dissolved. Each case body was relocated verbatim to a module-scope route*Command function via a brace-matching extractor; inner break; statements (from _dispatchNonFamily early-exit patterns) were converted to return; (5 arms affected); loop break; statements preserved. chore(#2371): representative gate-fixture corpus + document-shaped property test (#2380)
* chore(#2371): representative gate-fixture corpus + document-shaped property test Adds tests/fixtures/representative/ — a permanent corpus of verbatim, incident-sourced fixtures (never author-invented) from #2286, #2347, #2365, #2366, each labeled with its expected gate verdict in a MANIFEST.json and driven through the real CLI gate entrypoint via tests/representative-corpus.test.cjs. Adds a document-shaped fast-check property test alongside the existing writer-seeded bijection test in tests/api-coverage.test.cjs: the existing generator produces rows and renders them through the writer, so the document shape is a constant and it cannot fail against a decoy table; the new one generates the document space instead. Two gates (#2365, #2347) are still open, so their corpus/property assertions are marked with node:test's official `todo` option — the test executes and reports its failure without affecting the process exit code (https://nodejs.org/api/test.html#test-options). The audit-uat corpus (#2286, fixed by #2317) is a normal passing assertion, proving the methodology works end to end and not just cataloguing gaps. Records the fixture-provenance rule in CONTRIBUTING.md: a gate's fixtures may not be derived from the gate's own writer, grammar, or docstring examples; a negative fixture must come from a source that doesn't know the gate exists. No production src/*.cts changes — validation only, per #2371's scope. Closes #2371 Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#2371): address orthogonal-review findings — dedup generators, fix field naming, wire dead fields Standards-axis review findings, all fixed: - Deduplicated the row-shape generators (capabilityGen/rowGen/validRowGen) that were copy-pasted between the parse/render bijection test and the new document-shaped property test in tests/api-coverage.test.cjs — a future edit to one could have silently desynced the two properties. Hoisted to a single module-scope declaration both tests reference. - Renamed decision-coverage-guard/MANIFEST.json's expectedOutcome -> expectedReason. It asserted against the gate's `reason` field, but this codebase already has a real, different `outcome` field at parser altitude (extractDecisions' DecisionOutcome) — naming the manifest field after the wrong altitude's term was exactly the ambiguity the "Fixture provenance" rule this PR adds exists to eliminate. - Removed the unused `role` field from three MANIFEST.json files (never read by any test) and wired the previously-dead per-fixture `expectedMinItems` in audit-uat/MANIFEST.json into a real per-file assertion in tests/representative-corpus.test.cjs, using cmdAuditUat's `results` array — catches a regression that moves items between the two fixture files while preserving the aggregate total, which the existing total_items check alone would miss. No changes to test intent or coverage — same assertions, correctly named and fully wired. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix: remove dead capabilityState/capabilityWriter requires from gsd-tools.cjs Surfaced by the mandatory pre-PR lint gate (no-unused-vars) while preparing this PR — unrelated to #2371's own changes, but a defect found while working is fixed in place rather than deferred. Leftover from #2368/#2370 (merged just before this branch rebased onto it): the case 'capability' arm that needed these two requires was relocated to bin/lib/capability-command-router.cjs, which already requires both modules directly (lines 24-25) and is their only real consumer (cmdCapabilityState, resolveCapabilityRuntimeState, cmdCapabilitySet). The two requires left behind in gsd-tools.cjs had zero other references in the file and were never re-exported — confirmed via grep across the file and its module.exports. Behavior-preserving: Node's require cache means the underlying modules still load exactly once via capability-command-router.cjs's own requires; gsd-tools.cjs never used its now-removed local bindings. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#2371): replace todo-marked assertions with characterization tests gsd-test's own JSONL result parser (gsd-test-runner's internal/pipeline/parse.go, verified directly against that repo's source) has no concept of node:test's `todo` option — it only recognizes kind:"pass"|"fail" and hard-errors on anything else. A { todo: true } test whose body throws is counted as a real failure in gsd-test's own verdict, exactly as if it weren't marked todo — proven by an actual gsd-test run against this branch, which reported outcome:"failed" with all six todo-marked assertions (the property test plus five representative-corpus fixtures) in the failure list, each carrying the correct raw node:test `todo` field the tool's parser simply doesn't read. Replaces todo with characterization: MANIFEST.json now carries both the correct target verdict (expected*) and the exact current observed verdict (currentBuggyOutput, directly verified against live CLI output for all five fixtures). Tests assert currentBuggyOutput — an honest, non-vacuous pin of today's known-broken reality that passes today and will fail loudly the moment the referenced fix changes the observed output, at which point the assertion should be flipped to expected* and currentBuggyOutput deleted. The document-shaped property test switches from throwing fc.assert to non-throwing fc.check (returns RunDetails per fast-check's own docs) and asserts report.failed === true directly, for the same reason. Updates all prose (CONTRIBUTING.md, the fixture READMEs) that previously claimed todo would be respected — that claim was factually wrong for this repo's actual tooling and must not ship. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * chore: regenerate golden-install-parity fixtures after rebase onto next Rebasing onto the current next (which now includes #2381's todo-severity changes to gsd-core/bin/gsd-tools.cjs) produced real conflicts in all 18 golden-install-parity fixtures — expected, since both branches changed the same gsd-tools.cjs hash entry. Resolved by taking one side to unblock the rebase, then regenerating fresh from source via npm run gen:golden and verifying the result; every file's diff is exactly the one hash line for gsd-core/bin/gsd-tools.cjs, correcting a stale intermediate hash from the arbitrary conflict pick. refactor(#2368): extract capability arm to capability-command-router (ADR-2346 P2) (#2370)
* refactor(#2368): extract capability arm to capability-command-router (ADR-2346 P2) Behavior-preserving relocation of the 706-line case 'capability': arm from gsd-tools.cjs into a new hand-authored bin/lib/capability-command-router.cjs (sibling of ensure-runtime-build.cjs). The 15 bin/-relative require paths are rewritten to sibling-relative (correct for bin/lib/). dispatchHostCommand is now async (capability's install/upgrade/consent ops await the lifecycle); sync routers (state/phase/…) pass through await unchanged. case 'capability': removed; capability dispatches via HOST_COMMAND_ROUTERS. Validated by the existing capability-lifecycle / -consent / -trust / -loader test suites (no logic changed). Golden install-parity fixtures regenerated. Closes #2368 (Slice 1 — relocation). Probe consolidation (capHostVersion→ readHostVersion, capReadStrict dedup) deferred to a follow-up slice. * fix(#2368): add capabilityState/capabilityWriter requires + INVENTORY row The relocated capability arm references capabilityState (cmdCapabilityState, resolveCapabilityRuntimeState) and capabilityWriter (cmdCapabilitySet) — both module-scope requires in gsd-tools.cjs (L288/289) that the initial closure-dep scan missed. Added as sibling requires to capability-command-router.cjs. Also adds the new cli module to docs/INVENTORY.md + regenerates the manifest. * fix(#2368): correct capHostVersion __dirname depth for bin/lib/ relocation capHostVersion's VERSION/package.json paths were bin/-relative ('..' and '..','..'); on relocation to bin/lib/ they resolved one level too deep, so capHostVersion returned 0.0.0 and capability install failed the engines.gsd gate (#1920). Added one more '..' to each (now resolves gsd-core/VERSION and repo-root package.json correctly). * test(#2368): drop capability from the invocation loop (async/FS vs /fake/cwd) capability is async and does FS/config reads, so invoking it against the unit test's /fake/cwd is fragile. The 6 sync Tier-1 routers stay in the invocation loop; capability is covered by the non-invoking registry- ownership assertion + the dedicated capability-* test suites. * chore: retrigger CI (no-changelog label now present) |

refactor(#2360): host dispatch table + state cutover pilot (ADR-2346 P1) (#2364)
* refactor(#2360): host dispatch table + state cutover pilot (ADR-2346 P1) Pilot cutover for ADR-2346 Phase 1 (epic #2345). Introduces the Layer-2 host dispatch table — dispatchHostCommand + HOST_COMMAND_ROUTERS, consulted in runCommand's default case after capability/overlay dispatch, before the unknown-command error. Migrates 'state' as the pilot: removes the hardcoded case 'state': arm; state now dispatches default -> dispatchHostCommand -> routeStateCommand, byte-identical to the old path (proven by the new state-command-cutover equivalence test, 5-category template). Host commands are NOT capabilities (core, non-toggleable, no tier/activationKey) — the capability registry stays reserved for toggleable feature bundles per ADR-959. This is the host-vs-capability distinction the merged ADR-2346 lacked; the ADR is corrected here alongside the code that realizes it. - gsd-core/bin/gsd-tools.cjs: HOST_COMMAND_ROUTERS + dispatchHostCommand (prototype-pollution-safe); wired into default case; case 'state': removed; dispatchHostCommand + HOST_COMMAND_ROUTERS exported for tests. - tests/state-command-cutover.test.cjs: UNIT/DISPATCH/BEHAVIOR/REGISTRY equivalence (recording-mock + runGsdTools end-to-end + pollution guard). - docs/adr/2346-*.md: refine Decision 1/2 to the host-table vs capability- registry model (correction that did not land in the merged #2355). Behavior-preserving. Subsequent P1b/c PRs migrate phase/init/roadmap/validate/ verify using this proven template. Closes #2360. * test(#2360): regenerate golden fixtures + allowlist for state cutover Bookkeeping for the gsd-tools.cjs change: npm run gen:golden regenerates the install-parity fixtures (gsd-tools.cjs content hash changed), and the new tests/state-command-cutover.test.cjs is added to the lint-test-file-count allowlist under the 'state' prefix. * refactor(#2360): migrate remaining Tier-1 routers (phase/init/roadmap/validate/verify) Completes P1: all 6 Tier-1 host routers now dispatch via HOST_COMMAND_ROUTERS (state landed in the pilot commit). init preserves its #1688 warnIfStaleBake pre-hook; validate binds the output emitter. Cutover test extended to assert all 6 are consumed + owned. Golden install-parity fixtures regenerated. |

chore(#2143): markdown table model + schema registry + fail-loud pilot — Phase 1 (#2248)
* chore(#2143): markdown table model + schema registry + fail-loud pilot — Phase 1 Phase 1 of epic #2143 (ADR-2143): consolidate markdown table parsing onto a canonical seam and migrate the pilot reader. - Add src/markdown-table.cts: parseMarkdownTable (GFM tables -> typed {columns, rows} addressed by column NAME; ragged rows are typed parse errors, not silent), a single-source TABLE_SCHEMAS registry (RoadmapProgress / RequirementsTraceability / QuickTasks / Security, with variants under one id), matchTableSchema, and findTableBySchema. Result<T> is scoped to this seam (distinct from the dispatch Result). - Migrate deriveProgressFromRoadmap (src/phase-lifecycle.cts) off the position-anchored regex to name-based resolution via the seam — fixes #2137 (the 5-column milestone-grouped Progress table previously returned all-null). - Add a schema-backed `gsd-tools quick-tasks-append` subcommand and route fast.md's log_to_state through it, retiring the inline `awk NF-2` column arithmetic — fixes #2133 (addresses #2012, #2119). Cell values are escaped (| and newlines) and the STATE.md read-modify-write is atomic under readModifyWriteStateMd (lost-update race, cf. #500/#905/#1230). - Writer/reader/template parity test guards TABLE_SCHEMAS against drift (ADR-2143 §3 Generative-Fix-Divergence). Registration: .gitignore, eslint.config.mjs, docs/INVENTORY.md + INVENTORY-MANIFEST.json, CONTEXT.md glossary, docs/CLI-TOOLS.md. Behaviour-preserving for the canonical 4-column Progress table; the named bugs are driven fail-first. Extend-never-mutate (ADR-2143 §2). Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * chore(#2242): backfill changeset PR number (#2248) Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#2242): escape backslash before pipe in markdown-table cell escaping CodeQL js/incomplete-sanitization (high): escapeCell escaped | -> \| but not the backslash itself. Now escapes \ -> \\ before | -> \|, and splitTableRow unescapes both \\ -> \ and \| -> | symmetrically so cell values (incl. literal backslashes) round-trip exactly. Added backslash round-trip tests. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#2242): read ROADMAP Progress table by column name — supersede #2168 ad-hoc scan Rebase reconciliation with #2168 (the tactical #2137 fix that marked itself "pending #2143"). deriveProgressFromRoadmap now resolves the Progress table via a new seam helper findTableWithColumns (first table whose header is a superset of Phase/Plans Complete/Status/Completed, any order, extra columns ignored) and reads cells by NAME — order/injection-invariant per ADR-2143 §3 — instead of the exact TABLE_SCHEMAS match. This satisfies #2168's column-invariance property test while staying seam-based and preserving its `## Progress` scoping (#2012/#1445). Ragged Progress tables now resolve to null (ADR-2143 fail-loud); updated the stale state.test.cjs assertion that predated the Phase-1 migration. fix(#2118): honor --dry-run in milestone complete with zero-mutation preview (#2155)
* fix(2118): honor --dry-run in milestone complete with zero-mutation preview milestone complete treated --dry-run as a no-op: the flag was neither parsed nor rejected, so a caller who expected a preview instead triggered the full destructive mutation (archive phases → move audit artifacts → rewrite STATE.md) with no way to back out. Fix (option 2 from the issue): add dryRun to MilestoneCompleteOptions, parse --dry-run in the dispatcher, and return a JSON preview plan (would_archive, would_update) after the read-only stats gathering but before any mutations. Also gated platformEnsureDir on !dryRun so the archive directory is not created during preview. 3 regression tests: no-mutation happy path, --no-archive-phases combo, and --force bypass combo. * docs(changeset): backfill PR number (#2155) * chore(#2118): regenerate pi/qwen golden fixtures after next merge The milestone --dry-run fix changes gsd-core/bin/gsd-tools.cjs; qwen.json (missed at authoring) and pi.json (added on next, never carried the fix) were the only two runtime fixtures still holding the pre-fix hash. All other runtimes already reflect the change. Regenerated via the sanctioned gen-golden-install-parity script. * fix(#2118): surface accomplishments in dry-run preview; fix --dry-run --raw Orthogonal review findings on the --dry-run preview: - The preview omitted the already-computed accomplishments (the primary MILESTONES.md content a real run writes); surface it as a top-level field, mirroring the real-run result. - `--dry-run --raw` discarded the structured payload and printed the literal string "dry-run"; drop the raw-value arg so --raw emits the full preview JSON, matching the real-run output() call. Adds tests: --dry-run --raw is parseable JSON, preview includes accomplishments, and --dry-run --force is proven zero-mutation. fix(#1857): test gates normalize to one-shot + bounded timeout (no watch-mode hang)
A GSD verification gate resolves a project's test command and runs it. vitest defaults to WATCH mode in an interactive TTY — exactly where a user runs `gsd-execute-phase` — so a resolved `npm test`/`pnpm test` backed by vitest never exited and the orchestrator waited indefinitely. Recovery needed the user to manually prompt "something blocking?". Fix — one shared helper + a bounded, surfacing timeout on the test-command gates: - New pure module src/normalize-test-command.cts + `gsd-tools query normalize-test-command` verb: rewrites a resolved command to a best-effort one-shot form (direct vitest → `vitest run`; jest `--watch` → `--watchAll=false`; a package-manager `test` script whose package.json runner is watch-vitest → `CI=true` prefix; handles `--dir`; already-one-shot commands unchanged — never double-flagged). Named `normalize-test-command` (not `test-*`) so the file does not match node --test's default `test-*` discovery glob. - The three gates that HUNG or silently-continued route through that ONE helper and bound execution with `timeout $(config-get workflow.test_gate_timeout)` (new config key, default 600s): the regression gate (extracted to execute-phase/steps/regression-gate.md since execute-phase.md is size-frozen — it shrank 93528→93132; ABORTS on exit 124), the post-merge gate, and the audit-fix gate (previously an UNBOUNDED `eval`). All name watch/dev mode on 124. - verify-phase's gate was ALREADY bounded (a fixed `timeout 300`, not a hang), so it only gains the normalizer (so a watch runner exits fast) + a watch-mode hint on 124, staying under its frozen 40960-byte tier cap. Security hardening (review): the normalizer only rewrites a runner named as a standalone command TOKEN (so `run-vitest.js`/`make test-vitest`/paths are never mangled), is length-capped and uses only linear-time split-based scanning (no super-linear backtracking on an adversarial `workflow.test_command`), and reads package.json only when it is a regular file (never blocks on a FIFO via `--dir`). Config key `workflow.test_gate_timeout` (seconds, default 600) registered in the schema manifest + templates/config.json + docs/CONFIGURATION.md (mirrors workflow.cross_ai_timeout). New module registered in .gitignore, eslint ignores, inventory manifest/index. All 16 golden-install-parity fixtures + workflow size baseline regenerated for the changed shipped files; bin/lib is excluded from the parity manifest. Tests: tests/normalize-test-command.test.cjs (normalizer units incl. security hardening) and tests/test-gate-watch-mode.test.cjs (the three core gates route through the shared helper + configured timeout + exit-124 watch-mode hint; verify-phase asserted as normalize-only/already-bounded). tests/execute-phase-active-flags.test.cjs repointed at the extracted step; tests/planner-language-regression.test.cjs allowlist comment updated. fix: third-party capability skills surface correctly (#2045)
A skills-only role:feature third-party capability installed 'active' but its skills never reached the runtime surface, capability enable/set rejected it as 'unknown capability', and capability list disagreed with capability state. Three defects, fix shape 1b (teach resolveSurface, no on-disk linking):
D1 (materialization): resolveSurface built the surfaced skills Set only from the on-disk manifest; third-party cap skills live at ~/.gsd/capabilities/<id>/skills/ and never entered the Set -> surfaced:false. Fix: union registry.capabilityClusters values into the Set in the full-profile branch (idempotent for first-party, additive for third-party; prototype-pollution guarded).
D2 (enable/set unknown): setCapabilityState validated the capId against the static first-party registry instead of the composed overlay-aware registry. Fix: validate against loadRegistry({includeInstalled}) like capability-state.cts does.
D3 (list vs state): capability list derived status purely from ledger existence, never surface composition. Fix: add a 'surfaced' field to list rows sourced from the same resolver capability state uses, so list and state agree.
Regression coverage: tests/issue-2045-third-party-skills-surface.test.cjs asserts all five acceptance criteria (resolveSurface union, state surfaced:true, enable/set not-unknown, list/state agreement, first-party + unknown-id regression). gsd-test: 23916/23916 green (linux-node22 + linux-node24).

fix(#2003): add --runtime override to capability state + loop render-hooks
resolveCapabilityRuntimeState derived the config dir from resolveRuntime(cwd) (GSD_RUNTIME -> config.runtime -> 'claude') when no --config-dir was passed, so a repo with persisted runtime:'codex' resolved the config dir to ~/.codex where the Claude skill isn't installed -> surfaced:false / hooks silently no-op when the operator drove from Claude Code. capability state and loop render-hooks parsed only --config-dir, never --runtime, so there was no way to assert the actually-active runtime. Add a runtimeOverride param to resolveCapabilityRuntimeState (canonicalized via runtime-name-policy so aliases like codex-app work); when present it short-circuits the persisted-runtime fallback and resolves getGlobalConfigDir for the explicit runtime. Thread --runtime through cmdCapabilityState and cmdLoopRenderHooks, and parse it in gsd-tools.cjs for both commands (dual --runtime X / --runtime=X form, mirroring --config-dir and the existing capability-set --runtime precedent). Help text updated. Without the override, behavior is byte-identical to today (regression-guarded). |

feat(#2002): self-healing runtime build for plugin-marketplace installs (#2036)
The compiled gsd-core/bin/lib/*.cjs modules are gitignored build artifacts (ADR-457) shipped prebuilt in the npm tarball. A Claude Code plugin-marketplace / git-clone install materializes the repo tree directly and never runs `npm run build:lib`, so those ~148 files are absent and every CLI command dies at load with `Cannot find module './lib/cli-exit.cjs'`. gsd-tools.cjs now calls ensureRuntimeBuild() before its ./lib requires: a new committed bin helper that compiles the tree once on demand (lock-guarded, incremental-cache-cleared, portable `node <tsc>` invocation) when the sentinel cli-exit.cjs is missing, and surfaces an actionable error when TypeScript is unavailable. The already-built npm path is a single fs.existsSync no-op. ADR-457 (gitignored, build-at-publish) is preserved; no release-pipeline change. Verified against a simulated marketplace checkout (148 build outputs removed): the CLI self-heals and the result is byte-identical to `npm run build:lib`. feat(#1787): add /gsd:next smart entry workflow (#1798)
* docs: design spec for /gsd smart-entry command
Hybrid approach porting gsd-pi's smart-entry wizard to gsd-core:
deterministic classifier (gsd-tools smart-entry --json) + markdown
command/workflow with AskUserQuestion + --text fallback. Routing-first
('what now?' menu), 10 situations redesigned for gsd-core's phase loop.
* feat: add /gsd-start smart-entry command
State-aware front door adapted from gsd-pi's smart-entry wizard,
redesigned for gsd-core's markdown-first, multi-runtime architecture.
- src/smart-entry.cts: deterministic situation classifier (no-project,
paused, blocked, verify-failed, needs-first-phase, planning, executing,
verify-pending, idle-stranded, complete, unknown). Reads STATE.md,
ROADMAP.md, git, and verify signals; emits JSON the workflow consumes.
- gsd-tools.cjs: wire case + help listing.
- commands/gsd/start.md + gsd-core/workflows/gsd.md: thin markdown
dispatcher presenting an AskUserQuestion menu (with --text fallback for
non-Claude runtimes) and dispatching to existing commands. Falls back
to /gsd:progress if detection is unavailable.
- help.md: document /gsd:start (parity with bug-2954).
- tests: smart-entry.unit.test.cjs (classifier behavior across all
situations + priority + JSON shape) and gsd-workflow.structure.test.cjs
(markdown-layer invariants + every emitted command resolves to a real
slash command).
Spec: docs/superpowers/specs/2026-06-27-gsd-smart-entry-design.md
Note: command-contract (ADR-0002) requires a gsd:* prefix, so the bare
/gsd from the spec surfaces as /gsd-start.
* refactor: rename smart-entry command to /gsd:next
Rename the command from /gsd:start to /gsd:next per feedback. The
command file is now commands/gsd/next.md (name: gsd:next) and the
backing workflow is gsd-core/workflows/smart-entry.md (named for the
smart-entry classifier and gsd-tools smart-entry subcommand; does not
collide with the existing workflows/next.md, which is the progress
--next sub-workflow). help.md and the spec updated to match.
All affected tests (188) pass; lint:ci clean.
* fix: smart-entry reads real STATE.md schema (nested progress YAML + body Phase field)
Codex review found the classifier misread this repo's own STATE.md: it
looked only for scalar current_phase/total_phases frontmatter and body
fields named 'Current Phase'/'Total Phases', but real STATE.md stores
the phase as body 'Phase: N' and total_phases/percent under a nested
'progress:' YAML object. Both came back null, so active projects
(e.g. this repo at Phase 3 / verifying) wrongly classified as
needs-first-phase.
- detectSignals now reads total_phases + percent from nested progress{}
first, then scalar fm, then body; current_phase falls back to the
body 'Phase:' field (parseProsePhaseField lineage).
- Add regression tests against the real schema (nested progress YAML +
body Phase field) covering verify-pending + executing situations.
Verified against this repo: now classifies verify-pending (was
needs-first-phase). Coverage 93.25% lines / 86.99% branches.
* fix(workflow): tiered fallback when gsd-tools is broken (not just smart-entry)
Live test exposed a self-defeating fallback: when smart-entry --json
failed because gsd-tools itself was broken (missing
markdown-sectionizer.cjs), the workflow fell back to /gsd:progress —
which also depends on gsd-tools and would dead-end too.
Replace the single /gsd:progress fallback with a tiered recovery:
1. Probe gsd_run state-snapshot. If it ALSO errors, the whole tool
layer is down — read .planning/STATE.md directly with the Read tool
and synthesize a minimal situation + actions menu so /gsd:next stays
useful. Surface a rebuild hint.
2. Only if smart-entry alone is missing (older gsd-core), fall back to
/gsd:progress as before.
Matches the direct-read resilience the live agent already did by hand.

Full CI caught that adding commands/gsd/next.md + gsd-core/workflows/smart-entry.md
left docs/INVENTORY-MANIFEST.json stale (not in the affected-test scope that
no-mistakes' test gate runs, so it surfaced in CI). Regenerated via
node scripts/gen-inventory-manifest.cjs --write; inventory-manifest-sync
test now passes.
* fix: add 'next' to core_loop cluster, update INVENTORY-MANIFEST, fix gates.md ref
Co-authored-by: Codesmith <codesmith-bot@users.noreply.github.com>
* fix: regenerate golden install parity fixtures for /gsd:next
Full CI (shard 3/3) caught that adding commands/gsd/next.md + the
smart-entry workflow/lib made the per-runtime golden install parity
fixtures stale across all 16 runtimes. Regenerated via
UPDATE_GOLDEN=1 node --test tests/golden-install-parity.test.cjs.
All 16 fixtures + inventory-manifest-sync now pass.
* Fix smart-entry verify-failed phase scoping and empty resolve shim step
Scope detectVerifyFailed to STATE.md's current phase so leftover higher
phase directories cannot force verify-failed routing. Move the gsd_run
shim resolver into the workflow resolve step so agents define gsd_run
before the detect step runs smart-entry.

that needed it (hermes), re-sorted INVENTORY-MANIFEST.json, and regenerated
the gsd-next / ns-workflow skill descriptions to match the command surface.

* refactor(#1787): delegate /gsd:next in-project advancement to gated /gsd:progress --next
Reconciles the /gsd:next smart-entry front door with the existing
/gsd:progress --next engine (davesienkowski review on PR #1798). The
classifier previously recommended /gsd:execute-phase directly for the
`executing` situation, bypassing workflows/next.md Route 0
(resume-incomplete-phase invariant, #160) and Gates 1-3 — reproducing the
duplication that got the old flat /gsd-next removed (#3054), plus a
correctness hazard (executing the recorded current phase while an earlier
phase is silently incomplete).
Now planning/executing/verify-pending recommend `/gsd:progress --next`
(single gated engine); the specific command stays an explicit secondary.
Off-path states (no-project, paused, blocked, verify-failed,
idle-stranded, complete) keep direct recommendations — smart-entry's
distinct value over --next. Adds docs/adr/1787-gsd-next-smart-entry.md and
a regression test locking the delegation contract.

* docs(#1787): avoid literal /gsd-next token in ADR (bug-3054 guard)
The repo-invariants #3054 guard bans the removed /gsd-next slash form in
docs surfaces. Refer to the removed command as `gsd-next` (prose) — the
historical reference is unchanged, just the banned token is dropped.

* chore: gitignore compiled host-integration-sdk + handshake-serialized .cjs
Pre-existing gap from #1683: these two src/*.cts modules compile to
gsd-core/bin/lib/*.cjs but were omitted from the per-file ignore list, so
`npm run build`/`npm test` left them as untracked build artifacts (dirty
tree + accidental-commit footgun). Adds them alongside their siblings
(host-integration.cjs, mcp-server.cjs, …). Found while finishing #1798.

* test(#1787): lock per-situation action invariants for all 11 situations + ADR typo
Adversarial-review follow-ups:
- Add a test asserting every situation's action set has exactly one
recommended action, 1-4 unique-id /gsd:* actions (previously the
one-recommended/1-4 invariant was only sampled for 6 of 11 situations).
- Fix ADR typo: /gsd-progress → /gsd:progress.

* fix(#1798): split oversized test chunks so a slow shard can't trip the per-chunk timeout
Root-cause of the intermittent `full test (windows-latest, 22, shard 1/3)`
failure. It was NOT a leaked handle (the runner's kill message guesses that,
but --test-force-exit already exits leaks cleanly). Diagnosis:
- Ran every shard-1/3 file WITHOUT --test-force-exit + a 45s kill-timer:
zero hangs, zero leaks — every file self-exits. So no leaked handle / hang.
- CI activity profile: output kept flowing (slowly) right up to the 600.0s
kill — a dead hang would go silent. => pure slowness.
- Per-file timing: install-minimal-hooks.test.cjs is a 4987-line / 250-case
consolidation file doing dozens of real installs — 41s even on a fast Mac
(much worse on the slow Windows I/O path), plus an install-heavy cluster.
Mechanism: MAX_FILES_PER_CHUNK=180 packed the whole ~171-file shard into ONE
`node --test` chunk, so the entire shard's wall-clock ran against a single
600s per-chunk backstop. On slow Windows runners that single chunk crossed
600s and was killed mid-run — an intermittent false-negative gate that also
hits `next` directly.
Fix: lower MAX_FILES_PER_CHUNK 180 -> 90 so each shard splits into ~2 chunks,
each with its own fresh 600s budget and a fresh node process (also relieves
per-process memory pressure). Verified locally: shard 1/3 now runs as
chunk 1/2 (90 files) + chunk 2/2 (81 files), 5323 tests, 0 fail. Also made the
timeout kill-message name slowness as a cause instead of asserting a leak, so
the next debugger isn't sent hunting a nonexistent handle leak.

fix(#1920): resolve host version from gsd-core/VERSION + ship capability generators (#1938)
The flattened install layout broke the third-party capability ecosystem in two
ways. Both are fixed at the host-version / installer boundary.
Gap 1 — host version read as 0.0.0. The running GSD version was resolved via
require('../../../package.json') (loader/source) and require('../../package.json')
(the gsd-tools CLI), which in the installed layout is the versionless CommonJS
marker → the fail-closed fallback reported 0.0.0, so `capability install` rejected
any manifest with a real engines.gsd range as "incompatible with GSD 0.0.0". For
runtimes that get no marker, and for local installs, that walked-up package.json
could even be the USER's own project, reporting a wrong version. Fix:
readHostVersion() (capability-loader.cts, capability-source.cts) and capHostVersion()
(gsd-tools.cjs) now prefer the authoritative gsd-core/VERSION the installer already
writes for EVERY runtime (mirrors resolveVersionFrom(), #1383), falling back to the
runtime-root package.json for the dev/source tree, then fail-closing. This fixes
every runtime and the actual `capability install` CLI path without touching the
marker package.json, so uninstall is unchanged (no data-loss surface).
Gap 2 — scripts/gen-capability-registry.cjs (+ its sibling
gen-loop-host-contract.cjs) were never copied by the installer, so the loader's
never-crash invariant discarded every overlay and fell back to the frozen
first-party registry (installed capabilities silently inert). Now copied,
uninstalled, and manifest-tracked exactly like fix-slash-commands.cjs (#1223).
Regenerates the 16 golden-install-parity fixtures to capture the two newly shipped
generator scripts and the gsd-tools.cjs change.
Regression tests (RED→GREEN): readHostVersion VERSION-first / fallback / fail-closed
resolution; an end-to-end `capability install` against a REAL installed layout
proving the engines gate sees the real host version, not 0.0.0; and a real-install
check that both generators are shipped and manifest-tracked.

feat(#1754): CLI version-skew detection — warn when a global install shadows project-local GSD (#1755)
* feat(#1754): CLI version-skew detection — warn when a global install shadows project-local GSD Addresses #1754 (approved-enhancement). Detects when the running gsd-tools.cjs is outside the project root while a project-local install exists — the shadowing scenario from #1748 where a stale global canary CLI (retired @gsd-build/sdk) silently overrides project-local GSD. Implementation (Node CLI entry-point, not shell snippet — avoids bloating 93 workflow files past their size caps): - src/cli-skew-check.cts: pure function checkCliSkew({resolvedPath, projectRoot, projectLocalExists}) → string|null. Compares paths via path.relative; returns a warning when the resolved CLI is outside the project root AND a project-local install exists. Includes @gsd-build/sdk removal hint when the path matches. No I/O (pure), no gsd-sdk literal (avoids bug-2801 lint). - gsd-core/bin/gsd-tools.cjs: wired at startup via the existing findProjectRoot resolver. Non-blocking (try/catch; advisory stderr warning, never gates). - eslint.config.mjs: registers the new ADR-457 generated artifact in the ignores. - tests: 6-case suite (skew/no-skew/legacy/normalization); all green. - Golden fixtures regenerated (UPDATE_GOLDEN=1) for the new compiled artifact. - docs/how-to/update-gsd.md: Diátaxis reference note for the skew warning. Full suite: 3354 pass, 0 regressions (1 pre-existing local AGENTS.md failure). lint:ci green. Closes #1754 * chore(#1754): backfill changeset pr placeholder * chore(#1754): regenerate INVENTORY-MANIFEST for the new cli-skew-check source module --------- Co-authored-by: review-bot <review-bot@gsd> |

feat(#1708): typed documentation-sourced #853 dispatch-flatten (ADR-1239 Phase B) (#1719)
* feat(#1708): typed documentation-sourced #853 dispatch-flatten Graduate the #853 orchestrator-backgrounding decision from a scattered RUNTIME==='codex' prose check to a typed, documentation-sourced engine decision. Adds a backgroundDispatch dispatch sub-axis (sourced per host: codex+cursor documented true, 9 documented false, 5 undocumented), shouldFlattenDispatch(dispatch) (inline UNLESS background && backgroundDispatch, fail-closed), and a gsd_run query dispatch-should-flatten the plan/execute workflows call. Cursor is newly background-eligible per its docs (inline->background) — a documentation-justified behavior change. No RUNTIME-name residue for this decision. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * docs(#1708): backgroundDispatch citations in matrix + CONTEXT note Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1708): address review findings on typed dispatch-flatten Code/adversarial review: convert the manager.md/autonomous.md Compound Action preamble from hardcoded 'On Codex' to FLATTEN-based branching (the handlers already use the query; the preamble contradicted them and was wrong for cursor); make shouldFlattenDispatch null-safe + type-honest (accepts raw 'undocumented' registry values); make backgroundDispatch a required descriptor field (matching its siblings, all 16 carry it); strengthen the config.runtime behavioral test; update the bug-853 prose-pin test + comment. Security review clean; Codex confirmed no fail-open. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * test(#1708): backfill backgroundDispatch in role:runtime test fixtures Making backgroundDispatch a required descriptor field broke role:runtime fixtures in capability-manifest-version/capability-registry/host-integration-descriptors tests that build a dispatch object without it (caught by full gsd-test, not scoped npm test). Backfill backgroundDispatch:false into the well-formed fixtures; the deliberately-malformed 'required-field' test fixture is left malformed by design. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * test(#1708): update fix-1521 dispatch-gating assertion to the FLATTEN gate fix-1521 pinned the codex-specific run_in_background prose that #1708 graduated to the typed dispatch-should-flatten/FLATTEN gate. Update its assertions to verify FLATTEN=false gating (not a runtime name) + that the old RUNTIME===codex gate is gone. Caught by full gsd-test. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * docs(#1708): add changeset for typed dispatch-flatten Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * chore(#1708): remove stray temp PR-body file Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * test(#1708): add issue ref to bug-853 allow-test-rule annotations ADR-456 requires every allow-test-rule exemption to carry a see #NNN reference; the source-text-is-the-product annotations added when migrating the prose assertions lacked it (lint-tests CI gate). Add (see #1708). Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> --------- Co-authored-by: Claude Opus 4.8 <noreply@anthropic.com> |

enhance(#1579): deterministic gsd-tools query eval.score verb (#1583)
* feat(#1579): deterministic gsd-tools query eval.score verb Split C of #1573 (pure code, lowest risk). Adds an eval.score query verb (coverage*0.6 + infra*0.4; bands 80/60/40) mirroring the verify.* chain; gsd-eval-auditor consumes it instead of doing weighted arithmetic in-prompt. Non-breaking — additive only. arXiv: 2601.15130 (Plausibility Trap/DPDM), 2507.10281 (Table Agent), 2508.15754 (TIR). * fix(#1579): address review — domain guard, property test, glossary, SKIP_ROOT, inventory/baseline - C3 input-domain: reject out-of-domain eval.score (require 0<=covered<=total; was emitting overall_score>100 / negatives) - C1 property test: add tests/eval.property.test.cjs (fast-check) — determinism, band monotonicity, [0,100] bounds, never-throws - C2 glossary: CONTEXT.md "Eval Scoring Module" entry (source-of-truth path + interface) - C4: add `eval` to SKIP_ROOT_RESOLUTION (pure arithmetic; no .planning/ access) - inventory: register generated eval.cjs/eval-command-router.cjs (INVENTORY-MANIFEST.json + INVENTORY.md rows) - size: regen agent-size baseline for gsd-eval-auditor (reused gsd_run shim + eval.score step) - eslint: ignore generated eval*.cjs (ADR-457 bin/lib migration coverage) * fix(#1579): register eval family in alias-drift gates Add EVAL_COMMAND_ALIASES/EVAL_SUBCOMMANDS to scripts/check-alias-drift.cjs families and to familyArrayKeys in the manifest-coverage test, so the eval family lands under the same drift guard as every sibling family (state/verify/init/phase/phases/validate/roadmap). Addresses trek-e review. check:alias-drift ok; feat-3251 coverage 9/9; eval suites 10/10. * docs(#1579): use half-open verdict band ranges in CLI-TOOLS overall_score is fractional and thresholds are >=80/>=60/>=40, so a score in [79,80) is correctly NEEDS WORK despite the old '60-79' label. Relabel bands as 60-<80 / 40-<60 / 0-<40 to match the code. Addresses trek-e nit. * fix(#1579): validate eval.score CLI inputs Reject unknown infra tokens and fractional counts, and pin the 80-point verdict boundary including rounding-before-banding behavior. |

feat(#1602): deterministic coverage-metadata UAT routing for verify-work
Add an optional structured `coverage:` block to SUMMARY.md frontmatter and a deterministic classifier that `verify-work` consumes to route deliverables to auto-pass vs human-UAT — replacing the rejected #1598/#1599 post-hoc heuristic. - New `src/coverage.cts` (→ bin/lib/coverage.cjs) parses the nested coverage block (extractFrontmatter can't — its `-` items are scalars-only; this is a focused parser, sibling of parseMustHavesBlock), validates each entry, and classifies into auto_passed vs present. Frozen MODE/PRESENT_REASON/ERROR_CODE typed-IR surface. Exposed via `uat classify-coverage --summary <f>`. - Auto-pass is the narrow proven case only: strict-boolean human_judgment:false AND non-empty all-`pass` verification AND zero validation errors. Everything else — judgment, empty/failing verification, malformed entry — routes to the human (fail-safe). A malformed block falls back to legacy prose extraction and surfaces an error; an absent block is byte-identical to pre-#1602. - execute-plan create_summary populates the block (fail-safe default human_judgment:true); verify-work extract_tests consumes it; create_uat_file marks auto-passed entries `source: automated`. - Templates (summary + 3 variants), CONTEXT.md predicate + glossary, INVENTORY, eslint/gitignore registration, and Diataxis docs (COMMANDS reference + USER-GUIDE explanation) updated. - Behavioral tests via the CLI (no source-grep); parser-robustness regressions for the null-entry/comment-header/mis-indent cases found in adversarial review. feat(#441): add /gsd-capture --list-seeds for seed listing and audit (#722)
* feat(#441): add /gsd-capture --list-seeds for seed listing and audit Seeds (.planning/seeds/SEED-NNN-slug.md) could only be created (--seed), enriched (--enrich), or auto-surfaced at /gsd-new-milestone. There was no way to browse or audit parked seeds on demand. This adds a read-only listing, following the established --list → workflow pattern (per the approved scope on - gsd-tools `list-seeds [status]` (cmdListSeeds in src/commands.cts): scans the seeds dir, returns { count, seeds[], summary } JSON with each seed's id, slug, status, scope, trigger_when, planted, title. Optional case-insensitive status filter. User-controlled content is sanitized (sanitizeForDisplay) and every path validated (requireSafePath); read-only. Independent of audit.scanSeeds, which only returns unimplemented seeds for the milestone surface. - /gsd-capture --list-seeds routes to a new read-only list-seeds workflow that renders the seed table. Closes #441 * chore(#441): point changeset fragment at PR #722 * test(#441): allowlist list-seeds test in prompt-injection scan The test asserts that list-seeds neutralizes injection payloads (<system>, [INST]) embedded in seed content, so the fixtures legitimately contain those patterns — same as the sibling security tests already on the allowlist. * fix(#441): use canonical /gsd:capture colon form in list-seeds workflow Claude-facing source (commands/, agents/, gsd-core/workflows/, ...) must use the /gsd:<cmd> colon form per ADR/CONTEXT.md; the hyphen /gsd-<cmd> form is retired there (enforced by bug-2543-gsd-slash-namespace.test.cjs). The new list-seeds workflow used the hyphen form. * docs(#441): sync help full.md + INVENTORY for --list-seeds Adds the --list-seeds entry to the help reference (help/modes/full.md, per bug-2954 argument-hint↔help parity) and registers the new list-seeds workflow in docs/INVENTORY.md (88→89) and the generated INVENTORY-MANIFEST.json. * docs(#441): add --list-seeds how-to + drop phantom statuses Addresses CHANGES_REQUESTED on PR #722 (two documentation blockers): - USER-GUIDE.md Seeds section (how-to): extend the task to cover auditing parked seeds on demand via --list-seeds, including the status filter — kept task-oriented per Diataxis how-to mode. - CLI-TOOLS.md (reference): drop phantom statuses implemented|rejected from the list-seeds filter vocabulary; the system only produces dormant|active|triggered (src/audit.cts scanSeeds). Reference must be factually accurate and complete. * fix(#441): guard non-scalar status frontmatter in cmdListSeeds A seed with a bare `status:` line (extractFrontmatter yields {}) or a `status: [a, b]` value (yields an array) crashed the whole audit list: `(fm.status || 'dormant').toLowerCase()` throws a TypeError on a non-string. Coerce every frontmatter read through a `fmStr` helper (mirrors the existing `typeof fm.id === 'string'` guard), so a non-scalar status falls back to dormant and non-scalar scope/trigger_when/title can no longer leak a raw array/object into the JSON contract. Title is now capped symmetrically. Adds regression coverage for empty and array `status:` and non-scalar fields. Refs #441 * docs(#441): align list-seeds workflow status vocabulary The load_seeds step listed `implemented` as an example status filter, but the real seed vocabulary is dormant|active|triggered (src/audit.cts scanSeeds); `implemented` has no producer. Matches the earlier CLI-TOOLS.md correction. Refs #441 * refactor(#441): extract pure deriveSeedIdentity; match raw status in list-seeds Pull the seed_id/slug derivation out of cmdListSeeds into a pure, exported deriveSeedIdentity(stem, rawFmId) so the parsing contract can be property-tested in-process (review minor #1). No behavior change. Filter comparison now matches the raw lowercased status (both sides already normalized) instead of sanitizeForDisplay(status); sanitization is for output, not matching (review nit #3). * test(#441): add fast-check property coverage and count=1 boundary for list-seeds Adds tests/list-seeds.property.test.cjs with four fast-check properties over deriveSeedIdentity (never-throws, string-only contract, canonical id->seed_id/slug invariant, filename-prefix fallback) per RULESET.TESTS.property-based-testing (review minor #1). Adds an N==1 status-filter boundary case to list-seeds.test.cjs (review minor #2). * chore(#441): sync runtime launcher snippet into list-seeds workflow Propagate the current _runtime-launcher.snippet.sh (with non-Claude runtime home probes) into the new list-seeds.md workflow via scripts/sync-runtime-launcher.cjs, satisfying bug-891 (E) propagation. * test(#441): record list-seeds.md in workflow size baseline (#1074) --------- Co-authored-by: Tom Boucher <trekkie@nomorestars.com> |

fix(#666): pr-branch silently ignored planning.sub_repos (#667)
* fix(pr-branch): handle sub_repos from config with git -C (#666) Adds a `handle_sub_repos` step between `detect_state` and `analyze_commits`. When `planning.sub_repos` is set in config, the workflow now: - Reads sub-repo paths via `gsd_run query config-get sub_repos` - Skips the step entirely when the list is empty/null/[] - Scans each repo with `git -C "$REPO" status --porcelain` - Offers the user all/select/skip choices - For selected repos: creates a PR branch, commits all staged/unstaged changes, pushes, and opens a companion PR via `gh pr create` All git commands use `git -C "$REPO"` — never `cd "$REPO"` — because shell state does not persist between agent-executed commands. Closes #666 Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * chore: update changeset pr number to 667 * fix(pr-branch): address maintainer review — correct seam, behavioral tests, robustness Resolves all three blockers and seven robustness issues raised in PR #667 review: Blockers: - Use `planning.sub_repos` (not top-level `sub_repos`) so config-get actually resolves - Replace prose grep test with behavioral fixture tests using runGsdTools + local bare repo - Extract sub-repo git work into new `cmdPrSubrepo` seam in src/commands.cts; never uses git add -A — stages explicit files only (universal-anti-patterns.md:44) Robustness: - Dirty-repo list persisted via mktemp/cat, not bash arrays (cross-block safe) - Branch name embeds repo slug (${CURRENT_BRANCH}-${REPO_SAFE}-pr) to avoid collision - push --set-upstream so gh pr create finds the branch - Sub-repo base branch resolved via ls-remote with fallback to repo's default branch - Remote slug parsed with /github\.com[:/]/ (handles SSH + HTTPS + .git-less URLs) - rollback() cleans up branch on any mid-sequence failure - node -e replaces jq (always available, no undeclared hard dep) Refs: #666 Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * fix(pr-branch): security guard, push timeout, rollback fix, porcelain fix Security (Blocker 1): - Use security.cjs validatePath() in cmdPrSubrepo for symlink-safe workspace containment check — rejects ../escape, absolute paths, and symlink traversal - Add negative regression test: '../escape' repo path must be rejected Robustness: - Push uses timeout: 60_000 ms (network op needs more than the 10 s default) - Capture prevBranchName before checkout -b so rollback uses explicit name instead of git checkout - (fails on fresh single-branch repos) - Porcelain path parse: line.trimStart().slice(2).trim() handles all XY combinations and the execGit global-trim edge case uniformly Tests: 17/17 pass, lint: 0 errors Refs: #666 Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * fix(pr-branch): move regression tests to commands.test.cjs, add core.quotePath=false - Move cmdPrSubrepo behavioral + workflow source-invariant tests from standalone bug-666-*.test.cjs into tests/commands.test.cjs under describe('pr-subrepo') per TESTING-SUITES.md policy (no new bug-* files). Adds allow-test-rule: source-text-is-the-product see #666 for the workflow-source-invariant suite. - Add -c core.quotePath=false to git status --porcelain call so non-ASCII filenames (e.g. café) are not C-escaped, keeping slice(2) parse correct. * fix(pr-branch): remove obsolete regression tests for sub-repos handling * fix(pr-branch): update workflow-size-baseline, add dirty-scan timeout - Regenerate tests/workflow-size-baseline.json for pr-branch.md growth (+handle_sub_repos step, +timeout addition). - Add { timeout: 10_000 } to the execFileSync git status --porcelain call in the handle_sub_repos dirty-scan (repo convention: every git subprocess is bounded, never hangs). Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * chore: regenerate INVENTORY-MANIFEST after rebase onto next Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * fix(#666): handle rename staging and split changedFiles from filesToStage For git mv renames, the old path no longer exists in the worktree after the move — staging it with git add fails. Split parsing into changedFiles (both paths, for result.files) and filesToStage (new path only for renames; old is already staged by git mv). Also adds porcelain tests for staged renames, non-ASCII filenames, and a fast-check property test. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * fix(#666): rollback on push failure in cmdPrSubrepo If push fails the branch only exists locally; rollback cleans it up so the sub-repo is not left in a half-committed state. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * fix(#666): do not rollback after commit on push failure; add push-fail regression test Post-commit push failures are network/auth/policy issues — the user's work is already committed on the local branch. Calling rollback() at that point force-deletes the only ref holding the commit (data loss). Leave the branch in place and emit a retry instruction instead. Adds a regression test (pre-receive hook that rejects all pushes) asserting the branch and commit survive a push rejection so the failure path stays covered going forward. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * chore: regenerate INVENTORY-MANIFEST after rebase onto next Rebased onto current next (#1267 retired core.cjs). Stale tsbuildinfo and a leftover bin/lib/core.cjs build artifact were masking the drift — wiped both, rebuilt clean, and regenerated the manifest. gen-inventory-manifest --check now exits 0. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * fix(#666): validate sub-repo paths before git invocation in pr-branch.md The handle_sub_repos workflow ran git -C on raw planning.sub_repos config values at two points before the pr-subrepo seam's validatePath guard ever ran: the dirty-scan detection (git status) and the base-branch resolution (git ls-remote / remote show). A traversal entry could point git outside the workspace; an embedded newline could inject a spurious record into the newline-joined dirty-file output and into the shell-interpolated commit message. Adds a containment check + character allowlist to the dirty-scan node script (reject before any execFileSync), and a defense-in-depth shell case guard on the same value before the second, independent git -C invocation in the base-branch resolution block. Adds a behavioral test that extracts and executes the actual shipped node script from pr-branch.md (not a mirror) against a real traversal target and an embedded-newline entry, asserting neither reaches git or the dirty-file output. Also updates the stale cmdPrSubrepo doc comment: push failures no longer delete the branch (see prior commit), only stage/commit failures do. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * test(#666): make sub-repo traversal scan test genuinely fail-first The outside repo's only change was an untracked file, which the ?? filter excludes — so the repo looked clean even with the guard removed, making the traversal assertion vacuous (it passed against a neutered guard). Commit the file first, then modify it, so the outside repo has a tracked dirty change: without the path guard it WOULD be reported dirty, so the test now fails-first. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#666): symlink-safe (realpath) sub-repo containment in pr-branch.md Finding A from re-review: the workflow guard used path.resolve, which only normalizes '..' textually and does not follow symlinks — so an in-tree symlink whose name has no '..' or '/' (e.g. "evil" -> /outside) passed both the charset filter and the resolve+startsWith check, letting git status / ls-remote / remote show run against a directory outside the workspace. The pr-subrepo seam already used fs.realpathSync (validatePath); this brings the workflow layer to parity. - dirty-scan: realpathSync the root once, and realpathSync each candidate before the containment check; skip on throw. - base-branch resolution: replace the weak `case *..*|/*` guard with a realpath containment check that yields a validated absolute SUB_REPO_DIR, and run git -C against that instead of re-concatenating $ROOT/$REPO_REL. - security test: add a symlink-escape entry and a positive control (legit in-root backend must still be reported). Confirmed fails-first — regressing the scan to path.resolve makes the symlink case leak. Also fixes a misleading-fallback minor: the workflow now checks the seam's exit status and skips the companion-PR step on failure, instead of printing "branch pushed, open PR manually" after a real stage/commit/push failure. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#666): harden pr-branch sub-repo flow against round-12 edge cases Pre-emptive hardening of the workflow changes from the symlink fix: - continue-outside-loop: the "skip companion PR on seam failure" block used a bash `continue`, but the per-sub-repo iteration is prose-driven (the agent loops, not a literal `for`), so `continue` would warn and no-op. Reframed as prose-gated control flow keyed on $SUBREPO_EXIT — no bash loop assumption. - Windows portability: the new symlink security case now degrades gracefully (try/catch around fs.symlinkSync; skip just the symlink assertion when symlink creation lacks privileges) so it doesn't hard-fail on Windows CI. Verified: seam exits 1 on error / 0 on success (error() → process.exit(1), propagated through the shim), so the $SUBREPO_EXIT check is meaningful; bash -n clean on the touched blocks; commands 156/156; lint:ci green; manifest in sync. feat(#1451): wire gsd capability install/update/remove/list/disable/enable management CLI (#1457)
* feat(#1451): wire gsd capability install/update/remove/list/disable/enable CLI ADR-1244 D5/D6: the management command was built as a library (capability-lifecycle.cjs install/upgrade/remove + capability-ledger.cjs) across Phases 3-5 but never wired to a user-facing command — gsd-tools.cjs 'capability' only handled state/set. This adds the six subcommands, dispatching to the existing lifecycle/ledger: - install <spec> [--integrity] [--scope global|project] [--yes] [--shared-file <rel>]… - update [<id>|--all] [--scope] [--yes] [--shared-file] (re-resolves recorded source) - remove <id> [--purge-data] [--scope] (first-party rejected) - list [--json] (first-party + overlay, both scopes, JSON array) - disable|enable <id> (activation-state alias of capability set --off/--on) Scope→runtimeDir mapping matches capability-loader exactly (global=$GSD_HOME||home, project=project root; caps at <root>/.gsd/capabilities/<id>, ledger at <root>/.gsd-capabilities.json). Consent is non-interactive: --yes grants; without it an executable install aborts after printing the disclosure and writes nothing. Best-effort reconcile before each mutation. Tests: tests/capability-cli.test.cjs (20 behavioral, real resolver via local specs, GSD_HOME-sandboxed) — install consent/block/usage matrix, list, update round-trip, remove round-trip + first-party guard, disable/enable, unknown subcommand. Docs: docs/reference/gsd-capability-command.md reconciled to the real surface (ledger paths, --shared-file, consent model, disable mechanism, outdated marked planned); docs/COMMANDS.md gains the gsd capability entry. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1451): resolve adversarial-review findings + root-cause the --raw silent-output bug Adversarial-review (Codex) fixes: - capReadStrict passes a malformed strict_known_registries value THROUGH so the trust gate fail-closes on it (was silently downgrading to permissive) - installCapability/upgradeCapability gain an expectedId guard + first-party-id rejection (capability-lifecycle.cts): an overlay can't shadow a first-party id, and 'update <id>' can't act on a different id if the recorded source was retargeted - capability update: prints the consent disclosure, exits non-zero on --all partial failure, no longer masks the resolved id - capability remove: ledger-first ordering so an overlay is removable even if it shadows a first-party name; first-party guard only fires for ids not in the ledger - gsd-capability-command.md: disable/enable doc corrected (registry-known ids; overlay toggle not yet wired through this path) Silent-output bug (root cause, not waved off as pre-existing): - captureStdoutSyncWrites buffered fd-1 output and DISCARDED it on the throw path — any --raw command that emitted a result/error envelope then threw (to set a non-zero exit) lost ALL of stdout. Now it flushes the captured buffer before re-throwing (exit code preserved). - cmdCapabilitySet threw via process.exit() (bypassing the capture wrapper entirely); now throws ExitError so the wrapper flushes — matches the repo's no-process-exit architecture. - Regression test: capability disable <unknown> --raw must emit the JSON error envelope on stdout. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1451): address adversarial-review R2 — shared-file confinement, MCP no-clobber, config fail-closed - confinedSharedFile(): realpath-confine every shared-config write/strip to the scope root (mirrors safeRmUnder), so a --shared-file whose parent is a symlink escaping the scope can't write outside it. - mcpServers shared edits: never overwrite an UNOWNED entry — a name collision with the user's (or another capability's) server is skipped, so install/remove can't silently clobber user MCP config (hooks already append; the map-keyed mcpServers path was the gap). - capReadStrict: a PRESENT-but-unparseable .planning/config.json now fails CLOSED (lockdown) instead of silently downgrading the strict_known_registries policy to permissive. - Tests: symlink-escape shared-file writes nothing outside scope; colliding user mcpServers entry preserved; unparseable config blocks an external install. capability suite 83/83, lint clean. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1451): address code-review — aborted-status robustness + coverage + project-scoped strict doc - install/update: handle an 'aborted' result independently of the requiresConsent flag so it can never fall through to the generic 'blocked: unknown reason' arm (aborted always means consent-needed per the lifecycle contract; latent today, hardened for future status additions). - Clarify capResolveScope comment (project scope === already-resolved cwd) and document that strict_known_registries is a PROJECT-scoped policy (read regardless of --scope; no machine-wide allowlist) in gsd-capability-command.md. - Tests: update --all over an empty ledger returns an empty result set (exit 0); a flag value that looks like another flag (--integrity --scope) is rejected, not swallowed. feat(#1434): registry-driven dispatch for third-party capabilities (ADR-1244 Phase 5) (#1450)
ADR-1244 Phase 5 (D7). dispatchOverlayCapabilityCommand in gsd-tools.cjs dispatches an installed third-party capability command family via loadRegistry({includeInstalled}), gated on a committed ledger entry (consent) and confined to the capability's install root (defaultRequireFromInstallRoot: bare-.cjs basename + realpath containment, rejects ../ traversal + symlink escape); same own-property/function/sync/ExitError guards as the first-party path. capability-loader records _overlay.commandRoots only for accepted overlay caps with a committed, structurally-valid ledger entry (fail closed). First-party graphify/intel/audit unchanged (already on the registry seam). 3 Codex rounds converged + /security-review (no HIGH) + /code-review (Approve); gsd-test green both platforms; CI green.
Closes #1434.

feat(#1355): detect-and-warn guard for claude-code agent-teams (#1371)
* feat(#1355): detect-and-warn guard for claude-code agent-teams GSD's multi-agent orchestration can stall under claude-code's experimental agent-teams (a subagent's completion fails to route to the orchestrator). Per the maintainer decision, the accepted scope is a read-only detector + one non-fatal warning — NOT the declined run_in_background/TaskOutput conversion. - New Teams Status Module (src/teams-status.cts → gsd-core/bin/lib/teams-status.cjs): pure resolveTeamsStatus({runtime, env}) + thin CLI cmdTeamsStatus reusing resolveRuntime. active = strictly-truthy env flag AND runtime === 'claude'. - Wire `gsd-tools query teams-status [--active]` (read-only; no capability registration needed — conformance gates govern features, not query commands). - One non-fatal warning in plan-phase.md before the first Agent spawn, gated on `query teams-status --active`; zero behavior change on non-claude/teams-off. - Hermeticity: clear CLAUDE_CODE_EXPERIMENTAL_AGENT_TEAMS in run-tests.cjs + SESSION_ENV_KEYS. Docs reference + CONTEXT.md glossary. Built lib gitignored. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * chore(#1355): add changeset for teams-detect guard Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * test(#1355): bump plan-phase.md workflow size baseline (+407B for teams warning) The non-fatal agent-teams warning block added to plan-phase.md grew it 92759 → 93166 bytes, past its committed per-file baseline ratchet. The growth is small, deliberate, and still well under the workflow tier hard cap. Regenerate the baseline via `npm run size:baseline` (only plan-phase.md changed). refactor(#1289): T5 — migrate the final idiom-hard callers off the core spine (#1290)
The last 4 core importers, migrated off non-destructure idioms:
- gsd-tools.cjs: core.{error,ERROR_REASON,setJsonErrorMode,output} -> io.cjs;
core.findProjectRoot -> project-root.cjs (lazy wrapper preserved); inline
resolveWorktreeRoot require -> worktree-safety.cjs
- audit-command-router: DI default `_core ?? core` -> `_core ?? io` (seam preserved)
- intel-command-router: DI default -> `{ output: io.output, timeAgo: coreUtils.timeAgo }` (seam preserved)
- check-command-router: io destructure -> io.cjs; dynamic core['planningDir']
-> planning-workspace, core['findPhaseInternal'] -> phase-locator (typed
imports, dropped the Record-cast bracket hack)
Allowlist is now EMPTY — NO file imports the core spine. core.cts re-exports
are dead weight; T-final deletes core.cts + remaining shim tests + the lint.
No behaviour change.

refactor(#1190): extract ADR-22 drift-guard decision logic into a testable seam (#1242)
* refactor(#1190): extract ADR-22 drift-guard decision logic into a testable seam ADR-22's severity mapping, authority auto-upgrade, and rung>=3 hard-block lived only as prose in plan-review-convergence.md — untestable. Extracted into src/plan-drift-guard.cts (pure: AUTHORITY_RUNGS, getEffectiveAuthority, classifyDriftSeverity) + a gsd-tools drift-guard CLI seam (authority/severity), and rewired the workflow to call the seam deterministically instead of reasoning the decision in prose. 47 unit/e2e/structural tests cover the full severity table, the grep->intel auto-upgrade, and the rung>=3 HIGH hard-block. feat(#1213): Capability State Writer — write-side inverse of the resolver (#1225)
* feat(#1213): Capability State Writer — write-side inverse of the resolver Adds src/capability-writer.cts (setCapabilityState + cmdCapabilitySet) and the `gsd-tools capability set` subcommand: the write-side inverse of the capability resolver (ADR-1213). One desired capability state projects onto the substrates — `enabled` drives the runtime surface (canonical on/off), `gates` drive federated config keys (hook granularity), install profile is a read-only floor — then re-resolves and reports divergence (assert-and-report), so "off means off" holds as a write-time invariant. Adds batched setConfigValues; routes gsd:settings capability hook-gates through the writer. Docs: CLI-TOOLS reference, how-to, ADR-1213, CONTEXT.md term. fix(#1146): single base-branch resolver across forking workflows (#1198)
* fix(#1146): single base-branch resolver across forking workflows Replaces duplicated per-workflow bash detection that silently fell through to :-main on repos where origin/HEAD is unset (git init+remote add+fetch without set-head, most CI checkouts, many worktrees). New CJS module git-base-branch.cjs exposes `gsd_run query git.base-branch` with full precedence ladder: git.base_branch config override → origin/HEAD symref → git remote show origin (authoritative) → local branch presence → "main". All git subprocesses bounded with timeouts; degrades gracefully. Wires execute-phase, quick, ship, complete-milestone, and pr-branch to the single resolver. Removes 14 lines of duplicated detection bash across the five workflows. Includes 7 behavioral tests covering the full precedence ladder including the key regression case (master repo, origin/HEAD unset → must return "master", NOT "main") and an anti-regression guard that fails if any workflow re-introduces the :-main/:-master fallback pattern. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com> * test(#1146): add tests for flat base_branch config key and both-branch tie-break Closes two mutation gaps identified in adversarial review: - A2: flat {base_branch: ...} at config root (legacy key form) was covered by code but unguarded against mutation of lines 74-75 in resolver - H: tier-4 tie-break when both main+master exist locally (main wins, per tryLocalBranch JSDoc) was documented but untested 9/9 tests pass. Adding a || fallback ladder (git symbolic-ref then echo main) keeps the unified resolver as primary in real workflows while letting the test harness succeed without gsd_run defined. Also propagates updated runtime-launcher preamble to pr-branch.md (added in origin/next MemPalace PR) and regenerates workflow-size-baseline.json. fix(#1145): implement query user-story.validate handler (#1193)
* fix(#1145): implement query user-story.validate handler `query user-story.validate` was a phantom command invoked by mvp-phase.md (line 102) and verify-work.md (line 170) but had no CJS handler. Every call exited with "Unknown command: user-story". The dotted-form dispatcher strips the `query` prefix, splits on `.`, yielding command='user-story' which fell through to the `default:` case with no registered capability handler. Adds a `case 'user-story':` handler inline in gsd-tools.cjs (same pattern as the #1140 fix in PR #1148). The handler: - Validates "As a [role], I want to [capability], so that [outcome]." - Uses \S anchors to require non-whitespace content in each slot (whitespace-only slots like "As a , I want to ..." now correctly return valid:false — found by adversarial Codex review) - Returns { valid: boolean, errors: string[], slots: {role, capability, outcome} | null } - Supports --pick valid for bare boolean output (verify-work.md usage) - Added 'user-story' to SKIP_ROOT_RESOLUTION (pure string validation) - Added 'user-story' to TOP_LEVEL_USAGE command list Regression tests added to tests/commands.test.cjs (per lint-regression- test-names policy; new bug-NNNN standalone files are banned). feat(#1169): complete ADR-857 phase 6 — migrate features to Capabilities, revive dead gates, harden conformance gate (#1183)
* test(#1168): make phase-6 gate un-gameable — reject empty stubs + require loop shrink The migration assertion previously checked only role==feature, so a registration-only stub (empty hooks, logic left inline) would turn the gate green while phase 6 stayed incomplete — the exact false-completion pattern this gate exists to prevent. Strengthen it: each ADR-named feature must OWN its behavior (>=1 hook, or a command family); and plan-phase.md/execute-phase.md must shrink strictly below their frozen pre-phase-6 sizes (94519/93166 LF bytes), which also defeats double-run gaming (declare a hook but keep the inline block -> file does not shrink -> red). Gate now 5 pass / 4 fail (orphaned execute:wave:post, empty/unregistered features, config-key leaks, no shrink). Green is now reachable only by REAL migration. Refs #1168, #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * feat(#1169): migrate gap-analysis to a Capability (plan:post gate) First real ADR-857 phase-6 migration (pattern-defining tracer). gap-analysis moves from an inline post_planning_gaps branch in plan-phase.md to a real plan:post gate Capability: - capabilities/gap-analysis/capability.json: role:feature, plan:post gate (when=workflow.post_planning_gaps, blocking:false advisory), OWNS workflow.post_planning_gaps (federated out of central schema). - plan-phase.md: inline config-get + gsd_run gap-analysis block replaced with a plan:post render-hooks call site dispatching the gate; file shrinks 94519->93279. - src/check-command-router.cts: cmdGapAnalysisPlanPost runs the real gap analysis via gap-checker. - post_planning_gaps removed from central manifest; resolves via federated config (default true preserved). - tests/post-planning-gaps-2493: re-pointed to assert capability ownership. Verified: gate 5 pass / 4 fail (gap-analysis cleared from migration, plan:post-orphan, config-leak, and plan-phase shrink checks); loadConfig still returns post_planning_gaps=true; check command runs real analysis; 392/392 in the config/registry/federation/router net. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * feat(#1169): migrate profile-pipeline to a command-family Capability ADR-857 Decision 7: profile-pipeline becomes a command-family Capability (like audit/intel/graphify). capabilities/profile-pipeline/capability.json declares an 8-command family (scan-sessions, extract-messages, profile-sample, write-profile, profile-questionnaire, generate-dev-preferences, generate-claude-profile, generate-claude-md) backed by a new gsd-core/bin/lib/profile-pipeline-command-router.cjs; the inline case arms are removed from gsd-tools.cjs. Owns profile-pipeline.enabled (federated). Verified: registry shows role:feature with commands.length=8; scan-sessions/profile-sample run live via the family; gate cleared profile-pipeline from the empty-stub failure (only tdd/schema-gate/drift remain); 296/296 registry+inventory+gsd-tools tests; lint 0 errors. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1167): wire execute:wave:post + implement ui.safety-gate check Revives the second dead gate from #1167: ui.gates@execute:wave:post was declared but never dispatched AND its check.query (ui.safety-gate) was unimplemented. Adds the per-wave execute:wave:post render-hooks call site in execute-phase.md (fires after each wave's merge/cleanup, before the next forks) and implements cmdUiSafetyGate (frontend + UI-SPEC aware, mirrors cmdUiPlanGate) in check-command-router. +17 regression tests. Verified: phase-6 orphaned-points conformance test now PASSES (gate 6 pass / 3 fail); ui-safety-gate routable in dot+hyphen forms; check-ui-safety-gate 17/17, check-ui-plan-gate 18/18; lint 0 errors. Refs #1167, #1168. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * feat(#1169): migrate drift (schema + codebase) to execute:wave:post gates Removes the inline schema_drift_gate + codebase_drift_gate steps (77 lines) from execute-phase.md; drift becomes a Capability with two execute:wave:post gates (verify.schema-drift blocking, verify.codebase-drift advisory) dispatched via the per-wave render-hooks call site. check-command-router routes verify.schema-drift / verify.codebase-drift to the real detectors. Federates workflow.drift_threshold / drift_action / schema_drift_gate out of central. Also fixes the execute:wave:post dispatch prose to run NON-blocking (advisory) gates too — the prior version only ran blocking gates, which would have silently dropped the codebase-drift advisory after its inline step was removed. Behavior preserved. Verified: gate 7 pass / 2 fail (drift cleared from stub + config-leak; execute-phase.md 92297 < 93166 frozen -> shrink passes); both drift checks run real detection; loadConfig defaults preserved (threshold=3, action=warn, gate=true); drift-detection 56/56 + schema-drift 34/34; lint 0 errors. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * feat(#1169): migrate tdd to a Capability (plan:pre contribution + execute:post gate) tdd becomes a real Capability: a plan:pre contribution injects the <tdd_mode_active> planner guidance (rendered from PLAN_PRE_HOOKS_JSON like security's contribution), and an execute:post gate (tdd.review-checkpoint, advisory) runs the real end-of-phase RED/GREEN review via a new check-command handler. Inline tdd_mode reads + the inline planner block + the tdd_review_checkpoint step are removed; workflow.tdd_mode is federated out of central. The MVP+TDD per-task RED-commit gate is preserved — TDD_MODE is now derived from the execute:post hooks (capId==tdd active), not an inline config-get. BEHAVIOR CHANGE (documented, not silent): the --tdd CLI flag now persists workflow.tdd_mode=true via config-set instead of being per-invocation. Rationale: tdd is now a config-toggled Capability, and env vars do not persist across the workflow's separate bash blocks (config does), so an ephemeral override isn't cleanly achievable; --tdd therefore enables the tdd capability, consistent with how all capabilities are toggled. Verified: gate 7 pass / 2 fail (tdd cleared from stub + config-leak; plan-phase + execute-phase both < frozen sizes); contribution injection + execute:post gate dispatch wired; MVP+TDD gate preserved; tdd.review-checkpoint runs real review; full unit suite 556/0; lint 0 errors. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * feat(#1169): migrate schema-gate to a plan:pre contribution Capability The plan-time schema-push detection (former plan-phase.md §5.7) becomes a schema-gate Capability: a plan:pre contribution (into:planner, when:workflow.schema_push_detection) whose fragment carries the full ORM-detection + [BLOCKING] schema-push-task injection logic, rendered into the planner via the existing plan:pre render-hooks dispatch. The inline §5.7 block is removed (plan-phase.md 94519->90445). workflow.schema_push_detection is a new capability-owned (federated) key, default true. (The execute-side schema-drift gate was migrated separately into the drift capability.) Verified: registry inlines the fragment (len 2704) so it is actually delivered at plan:pre; gate 8 pass / 1 fail — all 5 ADR-named features now real Capabilities, only the config-leak test remains (intel/security, next unit). Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * feat(#1169): close the 3 capability config-key leaks — phase-6 gate now GREEN Removes the last inline config-get reads of capability-owned keys from plan-phase.md. security_asvs_level/security_block_on now flow through the security plan:pre contribution via a new loop-resolver configValues mechanism (resolves declared config keys with the same 4-level precedence as activation and attaches them to the rendered hook); the §5.55 banner reads them from PLAN_PRE_HOOKS_JSON. intel.enabled becomes a real intel plan:pre step (ref.command: intel api-surface) dispatched via render-hooks; the inline intel branch is gone. gen-capability-registry now validates ref.command as a third dispatch shape. Verified: phase-6 capstone conformance gate is FULLY GREEN (9/0); 3 leaks gone (grep=0); security configValues resolve to {2,medium}/default {1,high}; intel step present only when enabled; loop-render-hooks 62/0, capability-registry 287/0, capability-state/federated-config 113/0; lint 0 errors. Closes the migration half of #1169. Refs #1139, #1167, #1168. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1169): address adversarial review — restore schema-drift block, generic planner injection, uniform gate contract Adversarial review caught 2 real regressions the green gate missed: (1) schema-drift no longer blocked — the execute:wave:post dispatch read GATE_RESULT.block but verify.schema-drift emitted drift_detected/blocking, and onError:skip wrongly bypassed positive blocks; (2) only tdd's plan:pre contribution was injected into the planner, dropping schema-gate's schema-push detection and security's threat-model guidance. Fixes: (A) every gate check returns a uniform boolean 'block' under --raw (the dispatch form), with advisory gates (tdd/gap) carrying their report in 'message'; (B) gate-dispatch contract corrected at all sites — onError governs command errors only, a blocking gate's positive block always halts; (C) generic planner injection of all plan:pre contributions where into=='planner' (tdd + schema-gate + security incl configValues); (D) two new conformance assertions: planner contributions injected generically + every gate check.query returns boolean block under --raw. Verified: gate 11/11; all 6 gate checks return boolean block under --raw; full suite 595/0; lint 0 errors. Refs #1167, #1168, #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1169): restore MVP+TDD end-of-phase blocking escalation (2nd adversarial pass) The migrated tdd execute:post gate is statically blocking:false, but the contract (references/execute-mvp-tdd.md + CONTEXT.md) requires the end-of-phase TDD review to ESCALATE from advisory to blocking when MVP_MODE && TDD_MODE && a TDD plan misses a RED/GREEN commit. The migration prose had downgraded this to a 'strong advisory recommendation' — silent loss of the blocking escalation. Restore it: the tdd-gate dispatch now refuses to mark the phase complete (Phase blocked message) under MVP+TDD when GATE_RESULT.block is true; advisory otherwise. Also strengthen tests/execute-mvp-tdd-gate.test.cjs: hasBlockingEscalation previously matched any line with 'blocking'+'mvp+tdd' (so 'advisory (blocking: false) ... under MVP+TDD' was a false green); now it requires the real refusal semantics ('refuse to mark the phase complete' / 'phase blocked'). Caught by 2nd adversarial review pass. Verified: execute-phase.md 92702 < 93166 frozen; mvp-tdd-gate + phase-6 gate 19/0; full suite green; lint 0 errors. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1169): restore MVP+TDD proceed-block, codebase auto-remap, schema skip-flag (3rd adversarial pass) 3rd adversarial pass found 4 more silent regressions: (1) the tdd MVP+TDD 'refuse to mark complete' was nullified by a downstream 'ALWAYS proceed regardless of gate results' line — proceed is now conditional (stops on an active MVP+TDD block); (2) the test now asserts the proceed is NOT an unconditional override; (3) codebase-drift auto-remap (spawn gsd-codebase-mapper when drift_action=auto-remap) was dropped — the execute:wave:post advisory dispatch now consumes spawn_mapper/directive; (4) GSD_SKIP_SCHEMA_CHECK bypass was lost from the gate path — cmdVerifySchemaDrift now honors the env var (block:false when set). Verified: no unconditional proceed; GSD_SKIP_SCHEMA_CHECK=true -> block:false; gate 11/11 + mvp-tdd 9/9; full suite 569/0; lint 0; execute-phase.md 93109 < 93166. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1169): init.cts reads federated config keys from nested path (4th adversarial pass) Config federation moved tdd_mode/research/nyquist_validation from flat config.<key> to nested config.workflow.<key>, but src/init.cts still read them flat — so init.plan-phase/init.execute-phase emitted tdd_mode:false / research_enabled:undefined / nyquist:undefined regardless of config (a public command-contract regression; the migrated loops use render-hooks so enforcement was unaffected). Read via config.workflow (type-safe Record cast). Now init reflects the same resolved values + federated defaults (research/nyquist default true) as the render-hooks path. Verified: build clean; init.plan-phase emits tdd_mode:true/research:false/nyquist:false for set config, defaults true for empty; full suite 591/0; lint 0. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * docs(#1169): add changeset for ADR-857 phase-6 completion (PR #1183) Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1169): complete phase-6 migration fallout — restore TEXT_MODE, fix registry .claude leak, re-point stale workflow-contract tests The capability migration left real regressions and stale consumer tests that the per-module unit suite missed but the full cross-platform suite caught (27 failing tests): Real source regressions (fixed): - execute-phase.md lost its AskUserQuestion TEXT_MODE plain-text fallback when the inline schema_drift_gate step was removed — non-Claude runtimes would stall. Restored, and the execute:post gate-dispatch prose de-duplicated to cite the execute:wave:post contract (loop body shrinks below the frozen pre-phase-6 ceiling while keeping every onError/blocking nuance). - capabilities/tdd inline fragment hardcoded `@~/.claude/gsd-core/references/tdd.md`, baked verbatim into the committed capability-registry.cjs and leaked the install path on 11 non-Claude runtimes (registry .cjs is copied, not path-converted). Made the fragment path-free; regenerated the registry. The phase-6 conformance gate now guards this (no ~/.claude install path in any capability source or the generated registry). - plan-phase.md: removed a §5.7 stub re-added in error and routed Branch 2 to step 6 (schema-gate is a plan:pre capability, §5.7 is gone). Stale workflow-contract tests re-pointed to the capability dispatch they now must assert (behavior verified preserved in source first, assertions kept equal-or-stronger): bug-621 + bug-2851 (gap-analysis via gsd_run render-hooks plan:post + registry binding), feat-2527 (tdd_mode federated out of central), phase6-planning + plan-phase-ui-redirect (§5.6 bounded by ## 6.), plan-phase-drift-guard (intel when:intel.enabled skip branch). profile-pipeline-command-router.cjs un-ignored from eslint (hand-written, no TS source) + stale disable comments removed. Size baseline regenerated. Verified: full suite 15140 tests / 0 fail; lint 0 errors; conformance gate green legitimately. Refs #1139, #1167, #1168, #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * test(#1169): add ADR-857 E2E content-test coverage for the 12 loop points + capability deliverables Grounds the capability engine in behavioral E2E tests (drive the real render-hooks/check CLI + the real registry, assert typed result content — no source-grep), structured around what ADR-857 says to deliver. 207 tests; each genuineness-checked (flip the expectation, confirm it fails). Per-loop-point dispatch (7 files): empty-point negative-space across the 6 no-hook points; verify:post 3-step resolution+ordering+onError; plan:pre contribution/configValues + ui.plan-gate + intel; plan:post gap-analysis; execute:wave:post drift+ui gates via the check route (schema-drift block/skip, codebase-drift threshold BVA, auto-remap); execute:post tdd.review-checkpoint RED/GREEN; ship:pre security gate resolution + frontmatter-get predicate pieces. ADR-deliverable coverage (4 files): predicate boundary held (edge/prohibition probes stay core, not off-by-default Feature Capabilities — phase-6 exception); core loop runs with zero capabilities (all 12 points empty, init bundles resolve); contribution merge (multiple ordered <contribution from=> blocks); federated-config key removal on uninstall. federated-config allowlisted for its 3-file split (unit + integration + lifecycle). Refs #1139, #1167, #1168, #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1169): remove dead drifted converter dups + address adversarial review Lint cleanup (root-caused, not waved off): src/runtime-artifact-conversion.cts carried 11 agent-converter functions (+5 orphaned consts/helpers) that were never exported, never called, and had silently DRIFTED from the live hand-authored copies in bin/install.js (one even referenced an undefined `claudeToCopilotTools`). Deleted the dead duplicates; install.js's live copies are untouched (it never imported these). Lint now 0 errors / 0 warnings. Adversarial-review (Codex) findings fixed: - HIGH: execute-phase.md TDD_MODE used `jq ... || echo false`, silently disabling the MVP+TDD blocking gate on jq-less runtimes. Reverted to the `node -e` form (node is guaranteed; matches the file's other node-e usages) so a missing optional tool can no longer fail-open a blocking safety path. - MEDIUM: federated-config-key-removal orphan-key test was vacuous (it skipped the orphan assertion). Now asserts the removed capability's key is genuinely not surfaced/validated after uninstall. - LOW: phase-6 conformance leak regex broadened to catch absolute-home and Windows-backslash `.claude/(gsd-core|commands|agents|hooks)` paths, not only `~`/`$HOME` forward-slash forms. - LOW: bug-2851 plan:post dispatch assertion now requires `--raw` (matched its stated contract). - nit: plan-pre intel-step test duplicate assertion replaced with a distinct structured-output check. Size baseline regenerated (execute-phase.md 93089 < 93166 frozen). Refs #1167, #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * test(#1169): make runtime-homes-descriptor-drive titles environment-independent The descriptor-equivalence test embedded the absolute golden config path (`os.homedir()`-derived) directly in each `test(...)` title, so titles differed between macOS (`/Users/x/.claude`) and Docker (`/home/gsdtest/.claude`). Every test PASSES on both platforms (15885/0 leaf tests each), but gsd-test-summary compares results by title and reported 29+29 false "only in Mac / only in Docker" discrepancies for tests that actually pass everywhere. Move the golden path out of the title and into the assertion message (still shown on failure); titles are now byte-identical across platforms so the cross-platform comparator matches them. No assertion logic or golden values changed. Refs #1169. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#1169): derive TDD_MODE via gsd_run --active-cap, not node -e (fix prompt-injection CI gate) The prior fix reverted execute-phase.md:181 from jq to `node -e` to close a Codex HIGH (jq||echo-false silently disabling the MVP+TDD blocking gate on jq-less runtimes) — but the CI prompt-injection scanner BLOCKS new `node -e` in workflow markdown (inline code-exec = injection vector), turning the security gate red. Both forms were wrong: node -e fails the scanner; jq fail-opens a blocking safety gate; `config-get workflow.tdd_mode` is forbidden by the conformance leak gate (tdd_mode is capability-owned). Correct fix (what Codex recommended): a gsd_run-native boolean. Add an `--active-cap <capId>` flag to `loop render-hooks <point>` that resolves hooks the normal way and prints exactly `true`/`false` for whether a capId is active — scanner-safe (canonical launcher, no inline code), node-reliable (no optional jq to fail-open), and leak-free (render-hooks resolution, not config-get). execute-phase.md:181 now `TDD_MODE=$(gsd_run loop render-hooks execute:post --active-cap tdd)`. +5 behavioral tests for the flag. Verified: prompt-injection-scan --diff origin/next → 0 findings; conformance gate 13/13 (execute-phase.md 92934 < 93166); execute-mvp-tdd + tdd-mode + loop-render-hooks 87/0; lint 0/0. fix(#978): parse --force in milestone complete dispatcher so the guard's documented override works (#982)
* fix(#978): parse --force in milestone complete dispatcher so the guard's documented override works The dispatcher built `{ name, archivePhases }` but never parsed `--force`, so `options.force` was always `undefined` and the guard inside `cmdMilestoneComplete` (which tells users to "Re-run with --force to override") could never be bypassed. Add `const force = args.includes('--force')` and pass it into the options object. The guard already honors `options.force` — no changes to milestone.cts needed. feat(#985): intel command cutover — commands-only capability, last first-party family (ADR-857 phase 4d-impl-4) (#988)
* feat(#985): intel command cutover — commands-only capability, last first-party family (ADR-857 phase 4d-impl-4) Migrate the intel CLI command family from a hardcoded gsd-tools.cjs case arm to a registry-dispatched Capability (commandFamilies mechanism, #961), mirroring the graphify (#972) and audit (#984) cutovers. New src/intel-command-router.cts exports routeIntelCommand reproducing all 9 subcommands verbatim (incl. the status timeAgo non-raw post-processing), lazily requiring intel.cjs inside the route fn. capabilities/intel/capability.json declares the intel command family; commands-only (skills:[]), declares the existing intel.enabled gate (default false — behavior unchanged). Behavior-CHANGING (dispatch path) but equivalence-proven: CLI output identical; existing intel.test.cjs passes unchanged. Completes the first-party command- family cutover sequence (graphify/audit/intel). Closes #985 Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * test(#985): compute expected planningDir via path.join in intel cutover unit tests (Windows CI) The intel-command-cutover unit-mock assertions hardcoded a POSIX `/.planning` expectation while the router builds it with path.join(cwd, '.planning') → backslashes on Windows, so the planningDir-arg assertions (query/status/diff/ snapshot/validate/update/api-surface) failed only on windows-latest CI. Compute the expectation with path.join (cross-platform); production router unchanged. feat(#981): audit-uat + audit-open command cutover — commands-only capability (ADR-857 phase 4d-impl-3) (#984)
Migrate the audit-uat + audit-open CLI commands from hardcoded gsd-tools.cjs case arms to a registry-dispatched Capability (commandFamilies mechanism, #961), mirroring the graphify cutover (#972). New src/audit-command-router.cts exports routeAuditUat/routeAuditOpen, each lazily requiring only its backing module (uat.cjs/audit.cjs) inside the route fn — matching the old per-case lazy loads. capabilities/audit/capability.json declares the two command families; commands-only (skills:[]), no config gate, audit_review cluster untouched. Behavior-CHANGING (dispatch path) but equivalence-proven: CLI output identical; existing audit regression tests pass unchanged. feat(#972): graphify command cutover — first capability owning a command family (ADR-857 phase 4d-impl-2) (#975)
Migrate graphify into a Capability that owns its `graphify` command family, dispatched via the registry (#961 mechanism) instead of a hardcoded case. graphify is now an enable/disable plug-in. - src/graphify-command-router.cts: routeGraphifyCommand (standard route*Command), reproduces the removed case EXACTLY (query +--budget, status, diff, build, hidden build snapshot, usage/unknown errors); injectable _graphify test seam. - capabilities/graphify/capability.json: role feature, tier:full, skills:[graphify], config:{graphify.enabled default false}, commands:[{family:graphify, module, router:routeGraphifyCommand}]. - removed case 'graphify' from gsd-tools.cjs; graphify now flows default -> dispatchCapabilityCommand -> commandFamilies.graphify -> router. - regenerated registry (commandFamilies/bySkill/configSchema/profileMembership/ capabilityClusters for graphify); tier:full keeps 4c install/surface a no-op. Equivalence-proven: 8 recording-mock unit tests assert the exact fn+args per subcommand (budget, snapshot-vs-build); 9 subprocess tests assert distinguishing output shapes; existing graphify tests pass unchanged through the new path. Surfaced (not silently accepted): the pre-existing --budget-no-value NaN no-op quirk, preserved for equivalence, filed separately. feat(#961): capability command mechanism (commandFamilies index + default-case dispatch) — ADR-857 phase 4d-impl-1 (#964)
* feat(#961): capability command mechanism — commandFamilies index + default-case dispatch (ADR-857 phase 4d-impl-1) Build the capability command mechanism per ADR-959: the `commands` declaration field on the feature role, the registry commandFamilies index, and a real dispatchCapabilityCommand consulted in runCommand's default case (replacing the dead _dispatchNonFamily shim's role). The registry DISCOVERS a standard route*Command (no rebuilt handler table). On a default-case command, dispatch enforces a bare-.cjs-basename, resolves the module under gsd-core/bin/lib/, asserts confinement, requires the resolved path, and own-property-guards the router export before calling it. A require.main===module guard makes gsd-tools.cjs importable for tests; the CLI path is unchanged. Additive: commandFamilies is empty today, so the default case is behavior- preserving for every command; the 10 dead _dispatchNonFamily sites are untouched (future migration markers). The graphify cutover is the separate next step. Closes #961 Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com> * fix(#961): surface capability router failures structurally + enforce sync contract Review follow-up: dispatchCapabilityCommand now wraps the router invocation so an unexpected (non-ExitError) throw is converted to a structured, attributed error(msg, SDK_FAIL_FAST) — honoring --json-errors — instead of escaping as a raw stack trace; an intentional ExitError propagates unchanged. An async router (returns a thenable) is rejected loudly with a structured error (the contract is synchronous, like the 12 host routers). Not shipped as "consistent with existing behavior": the host's pre-existing version of this gap is filed as #965. feat(#945): unified capability-state resolver (ADR-857 phase 4b) (#946)
Add a read-side query composing the three toggle systems into one
per-capability view. resolveCapabilityState({registry, installedSkills,
surfacedSkills, config, cwd}) reports installed (skills ⊆ resolved install
profile), surfaced (skills ⊆ resolved surface), and per-hook active (no when →
active; non-empty-string when → resolved via _resolveActivationValue; empty/
non-string → inactive), with no forced composite verdict. cmdCapabilityState
does the I/O (resolveProfile + resolveSurface + loadConfig), resolves the
runtime config dir via the canonical getGlobalConfigDir (--config-dir override),
and surfaces resolution failures as warnings rather than a false installed='*'.
Routed as `gsd-tools capability state`.
Additive: install/surface/workflows untouched; consumed by nothing.
