b901d1e06f feat(#1953): complexity-triggered refactor extension point (execute:post) (#3261)
* test(#1953): failing-first suite for the complexity-triggered refactor hook

60 behavioral cases against src/complexity-trigger.cts, which does not exist yet:
decision-point counting, the comment/literal stripping leak surface, threshold and
jump-delta boundaries at limit-1/limit/limit+1, stable-anchor baseline semantics,
and fs fault injection via mock.method. Two fast-check properties assert that
stripping never manufactures a decision point and that comments and string
literals are score-neutral.

Also registers the refactor-trigger capability manifest (inert until
refactor.trigger_enabled) and regenerates the capability registry and matrix.

Verified RED on the remote runner before any implementation exists.

* feat(#1953): complexity-triggered refactor extension point

Adds the opt-in refactor-trigger capability. After a phase executes, an
execute:post step measures per-function complexity for the files the phase
touched and writes a scoped refactor proposal when a function crosses the
configured threshold or drifts past its recorded anchor.

Design notes worth carrying:

- The signal is computed in-core (decision-point counting over comment- and
  literal-stripped source, Node builtins only) rather than via Memtrace or a
  shelled-out analyzer. The hook fires as a deterministic CLI, not an agent
  with MCP tools, and core takes no external dependencies — this is the only
  option a behavioral test can bind to. The metric sits behind a seam.
- The baseline is a stable anchor, not a rolling value: set on first
  observation, moved only on disposition. A rolling baseline makes the delta
  the single-phase change, so a function creeping +2 per phase never trips a
  delta of 5 and the jump check adds nothing over the absolute threshold.
- Strict mode records an open deviation window in the broken-windows ledger
  rather than declaring its own ship:pre gate. ship.md has no generic ship:pre
  gate dispatch — only two hardcoded branches — so a third gate of any kind
  would be declared and never evaluated.
- The gate clears on the proposal being dispositioned, never on the score
  improving. A blocking complexity number is one an executor can satisfy by
  splitting a coherent function in two.

execute-phase.md gains a generic execute:post step-dispatch contract; it
previously matched only ref.skill == "code-review", so any other step
registered there was declared and never run. The code-review branch is
unchanged.

Full rationale in ADR-1953.

Closes #1953

* fix(#1953): close git option injection and symlink escape in the refactor hook

Three findings from the isolated security review, all fixed inline.

HIGH — changedFilesSince interpolated the --since value into a revision
token placed before the -- separator. A -- only stops PATHSPEC parsing of
arguments after it; git still option-parses what comes before. So
--since '--output=/tmp/x' became --output=/tmp/x..HEAD, which git accepts
as --output=<file> and uses to redirect diff output — an arbitrary write.
Fixed with --end-of-options before the revision range plus a conservative
ref validator. The validator deliberately permits ~ ^ @ { } because those
are legitimate git REVISION syntax (HEAD~1, main@{yesterday}) as distinct
from ref-NAME syntax; --end-of-options is the actual barrier. The doc
comment asserting the trailing -- was sufficient was wrong and is corrected.

MEDIUM — resolveConfinedPath confined by string prefix only, so a symlink
committed inside the repo passed the check (its own path is under cwd) and
readFileSync then followed it outside the root. Now lstat-checks for a
regular file and skips anything else with REFACTOR_FILE_UNREADABLE, so one
bad path skips one file and the run continues.

LOW — the new execute:post dispatch contract showed the gsd_run example
before the rule requiring ref.command be validated first. That prose is
executed by an agent, so textual order is execution order. Reordered.

* fix(#1953): make the analyzer able to see TypeScript at all

Found by running the shipped analyzer over its own source: it reported
functions=1 for a 940-line module with 24 function forms. A return-type
annotation or a generic parameter list made a function invisible —
`function f(a): number {}` and `function f<T>(a: T): T {}` both detected as
zero. Since gsd-core is written in .cts and the capability declares
.ts/.cts/.mts analyzable, the feature silently found nothing in this repo's
own primary language while reporting success. A safety net that reports
"all clear" because it cannot see is worse than no safety net.

All 98 tests passed over this, because every fixture was plain JS — the
exact failure the test matrix's own "assert against the shape production
uses" warning describes. Adds a TypeScript-shapes suite covering return
types (including unions, generics, object literals and type predicates),
generic parameter lists (constrained and defaulted), export/async/generator
combinations, annotated arrows, class-method modifiers, and optional/
default/rest params — plus the two traps: an overload signature has no body
and must not count, and `a < b && c > d` is a comparison, not a generic.
Detection now reports 24/37/21 functions for the three source files, which
matches a hand count exactly.

Also from review:

- The strict-mode ledger dedup identified entries by parsing a prose
  description string. That is banned by CONTRIBUTING's raw-text-matching
  rule and was a real bug: the "exactly one window per untriaged proposal"
  guarantee rested on prose matching, so rewording a description or editing
  WINDOWS.md by hand silently produced duplicates. Now matches structurally
  on kind + phase + file + line.
- A property test asserted on the stripper's output text. Reframed to
  assert the same invariant through analyzeSource's score.
- nextBaseline's `candidates` parameter has been dead since the anchor
  change; removed from the signature and all call sites.
- Extracted the duplicated require-or-degrade and capability-check
  boilerplate.
- ADR-1953's Implementation bullet still named a `refactor.ship-gate` in
  check-command-router.cts — a leftover from the design cut D6 rejects.
  That file is untouched and no such gate exists. Removed.

* fix(#1953): keep execute-phase.md under its byte ceiling; un-vacuum the large-file test

Five of the seven remote-runner failures were one cause: the execute:post
dispatch contract, written out inline, grew execute-phase.md 1876 bytes
(93,400 -> 95,276) against a frozen PRE_PHASE6 ceiling of 93,600. A drift-ack
does not clear that — tests/phase6-capstone-conformance.test.cjs and
tests/fix-2285-claude-orchestration-wiring.test.cjs assert the file is
literally under the cap.

The contract now lives in gsd-core/references/loop-hook-dispatch.md, which
already claimed to be the point-agnostic dispatch reference and already
documented ref.skill and ref.agent. It gains the ref.command shape, its
in-context validation rule, the advisory-by-construction statement, and a
note that a point whose workflow hand-rolls one kind is not implementing
this contract. execute-phase.md now defers to it in one line: 145 bytes of
growth, 55 B of headroom under the cap. Better placement than the first cut
— the reference was overstating its coverage, and this makes the claim true
rather than duplicating prose next to it.

Acknowledged by appending to tests/emitted-drift-acks/2930-*.json rather
than a new 1953-*.json: two ack sources may never name the same path, and
that fragment is already the accumulating ack for this file.

Sixth and seventh failures: analyzesLargeFileWithinBounds tripped its own
vacuity guard — the fixture generated ~480 KB against a `> 500000` assert,
so the guard fired and the three assertions after it never ran. The test
has been vacuous since it was written. The matrix row specifies ~1 MB, so
N goes 8000 -> 20000 (1.17 MB, 17% margin) and the guard to > 1_000_000.
Verified by reproducing the exact body against the compiled module: 1168888
bytes, 118 ms, all four assertions hold.

Refs #1953

* fix(#1953): fold the execute:post step deferral into the existing resolve line

The remaining two failures were one test: execute-phase.md carries a SECOND,
tighter assertion than the 93,600 ceiling — `<=93400`, which is exactly its
current size. The file cannot grow by a single byte. My previous fix got it
under 93,600 but not under 93,400, so it still failed. ("H." in the report is
just the parent describe of that same test, not a separate defect.)

Rather than add a paragraph, the deferral now REPLACES the existing hook
resolution line. It read:

  Resolve active step hooks from `EXECUTE_POST_HOOKS_JSON` where
  `kind == "step"` and `ref.skill == "code-review"`.

which is the bug itself written down — only code-review was ever dispatched.
It now reads:

  Dispatch each `kind == "step"` hook per
  @gsd-core/references/loop-hook-dispatch.md. For `code-review`:

The following prose already begins "If no active code-review step hook
exists", so it reads correctly and the code-review handling is untouched.
Net effect on the file is -11 bytes: 93,400 -> 93,389, under the margin
assertion rather than merely under the ceiling.

That also removes the need for a drift-ack: the file shrank, so there is no
growth to acknowledge, and the append to the shared 2930-*.json fragment is
reverted. Leaving it would have shipped a claim of "145 bytes of growth"
that is no longer true, on a file six other issues share.

The test's own comment states the principle this ended up honoring: "the host
loop must stay small — optional-feature detail belongs in the capability
fragment, not the host workflow." Putting the dispatch contract in the
reference rather than inline is that rule, applied.

Refs #1953

* fix(#1953): keep the code-review hook literal the workflow test requires

tests/code-review.test.cjs extracts the <step name="code_review_gate"> block
and asserts it contains `ref.skill == "code-review"` verbatim. The previous
commit replaced the line carrying that literal, so the token vanished and the
test went red — a fair assertion: code-review IS the bespoke branch there and
the workflow should still name it.

Restored inside the same one-line deferral, which now reads:

  Dispatch `kind == "step"` hooks per @gsd-core/references/loop-hook-dispatch.md.
  `ref.skill == "code-review"`:

93,396 bytes — still under the `<=93400` margin assertion and 4 bytes below
the base, so the file continues to shrink rather than grow.

Because three consecutive runs were each reddened by a different assertion on
this one file, this change was verified by sweeping ALL of them at once rather
than one run at a time: every test under tests/ that reads execute-phase.md or
references/loop-hook-dispatch.md was located by resolving its path constants,
and each content/size assertion was evaluated directly against the working
tree — 22 assertions, plus two real executions (gen-section-manifest --check,
and emitted-attribution's full real-tree differential). All pass.

That sweep also confirms the earlier judgement call: the net change to
execute-phase.md is a SHRINK, and the size ratchet only gates growth, so
reverting the append to the shared 2930-*.json ack fragment was correct — an
ack would have been both unnecessary and factually wrong.

* docs(#1953): add the missing how-to for acting on a refactor proposal

Reference and explanation shipped (COMMANDS.md, CONFIGURATION.md,
FEATURES.md 159, ADR-1953) but the Diataxis how-to quadrant did not, and
that is the one a user reaches for. CONTRIBUTING's required-docs table is
'new command -> COMMANDS.md + FEATURES.md', so CI was green on a gap.

Enabling this feature is genuinely multi-step and no single page walked it:
turn it on, tune the threshold, understand advisory vs strict, discover
that strict needs a SECOND toggle on a DIFFERENT capability, and know what
to do when a proposal appears. The two-toggle subtlety in particular was a
footnote in a config table; here it is a section with both commands.

Follows the shape of its closest siblings, resolve-edge-coverage-findings
and resolve-prohibition-findings — both 'the loop surfaced a finding, here
is what to do with it'. Includes a reason-code table for the silent cases,
since the analyzer is deliberately quiet in six situations and a user who
expected a proposal needs to tell 'nothing to report' from 'could not look'.

Indexed from docs/README.md beside the other loop how-tos.

Docs-only: exempt from the push gate, no re-verification, pass marker on
2af188b4 untouched.

* feat(#1953): warn when strict mode is on but nothing will actually block

Closes acceptance criterion 5, which I had wrongly marked satisfied.

refactor.trigger_strict records an untriaged proposal as an open deviation
window, but a ship only STOPS if workflow.windows_enforce is also on — a
toggle owned by the broken-windows capability that this feature neither sets
nor requires. So a user could enable strict, believe ship was gated, and find
out otherwise at ship time.

The split itself stays: requires:["broken-windows"] would force-install the
ledger on advisory users who never enable strict, and a ship:pre gate of our
own would never fire because ship.md has no generic ship:pre gate dispatch.
What was missing was discoverability, so that is what this fixes.

`refactor evaluate` now emits a typed REFACTOR_STRICT_NOT_ENFORCING warning,
naming the exact remediation command, whenever strict is on and either
workflow.windows_enforce is off or broken-windows is unavailable. It fires
only on a run that produced a candidate — with nothing to block on there is
nothing to warn about, and warning every run would be noise.

Reads workflow.windows_enforce through the same resolveConfigKey walk the
router already uses for its own keys rather than a second config reader.
Four tests cover the matrix: strict+enforce-off warns, strict+enforce-on does
not, strict+ledger-absent warns, strict-off never warns.

Also corrects a user-facing message in this same file that told the user to
run `gsd-tools config-set` — the wrong form. docs/CONFIGURATION.md and the
broken-windows capability both use `gsd config-set`, and gsd-tools is invoked
as `node gsd-tools.cjs`, so the bare form may not resolve. The two adjacent
messages in this file now agree.

0c413bbc9c chore(#3059): close the ESLint glob-coverage escape and guard it (#3277)
* chore(#3059): close the ESLint glob-coverage escape and guard it

62 tracked source files matched no `files:` glob in eslint.config.mjs, so
ESLint skipped them entirely while `eslint .` still exited 0 — including all
26 files under hooks/, the enforcement machinery itself.

Covers 56 of them (eslint-rules/, hooks/, bin/lib/, pi/, examples/, vscode/,
the plugin shims, root *.mjs) and allowlists the 6 deliberate
must-not-compile brand-typing fixtures with a recorded reason each.

hooks/** is covered with n/no-process-exit deliberately off: a hook's whole
contract is its exit code, several exits are load-bearing stdin-timeout
guards where nothing else terminates the process, and ADR-0012/0174 scope the
no-process-exit convention to the Command Routing Hub. bin/lib/ui-safety-gate.cjs
is dual-mode, so it keeps the rule live and takes two targeted disables in its
require.main===module tail instead.

Adds scripts/lint-eslint-glob-coverage.cjs + a node:test drift guard so the
class cannot regrow: allowlist entries require a non-empty reason, the list
ratchets down only (a stale entry fails), and a tracked-count floor means a
broken `git ls-files` fails rather than reporting clean.

Closes #3059

* chore(#3059): apply review findings — correct the changeset count, add parser properties

Isolated adversarial review, confirmed by rebuilding a byte-for-byte replica
of the pre-change eslint.config.mjs: the changeset claimed 44 previously-
unlinted files. The real figure is 56 (56 covered + 6 allowlisted = 62).
That was user-facing CHANGELOG text and was wrong; corrected, along with three
consequential figures in the design record.

CLAUDE.md requires a fast-check property test for parsers and budget limits,
and listTrackedSourceFiles is a parser. The standards review called this
"satisfied in spirit"; it is not. Adds three properties driving the real
exported parser through an injected execFile: extension totality/soundness
including a trailing terminator, backslash-normalization totality, and
CRLF/LF equivalence — the invariant the repo's recurring CRLF defect class
breaks.

Also de-duplicates the anchor rows onto one shared resolver, kept deliberately
independent of the guard's own resolveFileCoverage so an anchor still fails if
that resolution regresses, and records in the guard's header why the
bin/install.js family is NOT allowlisted: it resolves to 2 rules under
ADR-1703, so an entry would trip the allowlist_stale ratchet.

* fix(#3059): make the coverage guard's git call container-safe

The remote runner reported the guard degrading to `git_failed` on both Node
lanes:

  fatal: detected dubious ownership in repository at '/work'

The runner executes in a container where the repo is owned by a different UID,
so git refuses to operate on it. The guard's degraded-verdict path worked
exactly as designed — it reported the failure instead of throwing or falsely
reporting clean — but a guard that cannot run in CI is not a gate.

`git ls-files` is now invoked as `git -c safe.directory=* ls-files`. `-c`
scopes the override to the single invocation and mutates no config file, and
the wildcard is appropriate because this command only enumerates tracked paths
in the repository it is already executing inside.

Adds a regression test that captures the argv through the injected execFile
seam and asserts `-c safe.directory=*` precedes `ls-files`, so the container
case is pinned behaviorally rather than by reading the script's source.

* chore(#3059): backfill changeset PR number

pr:0 placeholder replaced with the real number now that #3277 exists.

2ac21c7fdb docs(#2980): ratify the payload-carried error idiom as a degraded result (#3270)
* docs(#2980): ratify the payload-carried error idiom as a degraded result

Records ADR-2980: an `error` key in a gsd-tools result payload on stdout with
exit 0 is a ratified contract meaning the command ran to completion and is
reporting a condition, not a process failure. Faults keep stderr + exit 1 +
--json-errors.

Normalizing the 42 output({error}) sites to exit 1 was declined on measured
blast radius (get_impact rates cmdStateSnapshot CRITICAL; output has 170 direct
callers) — a Hyrum's Law break with no versioning escape hatch for an exit code.

Adds the "Degraded results vs faults" section to docs/json-errors.md with a
correct-caller recipe, indexes that page from docs/README.md, and records the
two-channel contract in the CONTEXT.md I/O Module entry. No code change.

Closes #2980

* docs(#2980): correct the site count and cross-refs after review

The isolated review found the population figure was the answer to a regex,
not to the question. `output\(\{\s*error:` only matches literals whose FIRST
key is `error`; re-deriving it by brace-matching output()'s first argument
gives 60 sites across 9 modules (42 error-first + 18 error-not-first), adding
workstream.cts, phase.cts and gsd2-import.cts. roadmap.cts:260 is the case in
point — it carries `error` alongside `found:false` and is the site that
actually produces the documented `roadmap get-phase` output.

Also from review: correct the --raw claim (11 sites pass a rawValue, not 2),
reconcile the missing-required-argument count to the 7 verified sites, link
the bare ADR-2966 references per the ADR lifecycle rule, and fix 'licence'
to American spelling.

a5706bd39d enhance(#2596): validate a wave branch's committed diff stays in its declared scope (#3264)
* test(#2596): failing-first suite for worktree-wave scope conformance

Binds the advisory diff-vs-declared-scope check to behavior before it exists:
the pure coverage predicate, the SUMMARY-artifact exemption and its parity with
the rescue walker, the gauntlet integration (never flips ok, degrades on a git
failure, survives a later block), the manifest normalizer's files_modified
handling, and the --files negative-input matrix on record-agent/create.

Refs #2596

* enhance(#2596): warn when a wave branch commits outside its declared scope

The worktree-wave merge gauntlet validated branch, base, deletions, SUMMARY
rescue and a clean worktree, but never compared a plan branch's actual
committed diff against the files_modified the plan declared — so an executor
that committed outside its brief merged into shared phase state silently.

Adds an advisory scope-conformance check: when the manifest entry carries a
declared scope, the gauntlet diffs HEAD...<branch> and appends one structured
warning per path outside it. It never flips ok and never blocks the merge;
promotion to a hard gate is a separate, disclosed change. With no declared
scope no git subprocess is spent at all.

Refs #2596

* docs(#2596): document the advisory worktree-wave scope-conformance check

Records the optional --files flag on worktree record-agent/create, the
advisory warnings channel cleanup-wave now emits, and its two deliberate
noise limits (SUMMARY-artifact exemption, literal-prefix glob matching).
Wires execute-phase to pass the plan's already-parsed PLAN_FILES.

Refs #2596

* fix(#2596): close review findings on the scope-conformance advisory

- share one path normalizer between the SUMMARY-artifact predicate and the
  scope comparison so the exemption and the check cannot drift
- wire --files into the orchestrator-worktree dispatch, which created a
  worktree but never declared its scope, so the advisory silently did not
  apply on that backend; ADR-1239 requires both adapters share one check
- correct the now-false blockquote claiming the check does not exist yet
- add the fast-check property tests the repo requires for parser logic
- add the record-agent/create parity test that Generative Fix Divergence
  requires for two surfaces implementing one rule

Refs #2596

* fix(#2596): keep execute-phase.md under the frozen pre-phase-6 byte ceiling

The one-sentence note added with the --files flag pushed execute-phase.md to
93708 bytes, past the ADR-857 PRE_PHASE6 cap of 93600 — the tightest of the
three workflow size gates, and a hard cap an acknowledgment cannot clear. It
failed three tests plus the differential attribution check.

Condense the note to a one-line pointer (93543, 57 B of headroom); the full
explanation already lives in docs/CLI-TOOLS.md and the dispatch step. The flag
itself stays in the command, because the orchestrator reads this workflow at
runtime and cannot pick it up from docs/.

Acknowledge the remaining 143 B of growth by appending to the existing
execute-phase.md fragment rather than adding a second one — the ack lint
rejects two sources naming the same path.

Refs #2596

* fix(#2596): make the execute-phase.md edit net-negative, not merely under the cap

The size gate on this file is two assertions, not one: bytes < 93600 AND
bytes <= 93400. The base is exactly 93400, so the file is at its budget and
any growth trips the margin assertion — the previous fix cleared the ceiling
but not that.

Move the --files explanation to per-plan-worktree-gate.md, which already owns
PLAN_FILES and carries no cap, and reclaim the rest from two clauses in the
sentence being edited: the cleanup-wave rules phrasing, and a 'non-zero exit'
the very next sentence already states. execute-phase.md ends at 93392, eight
bytes below base. The flag itself stays in the command — the orchestrator
reads this workflow at runtime and cannot pick it up from docs/.

With no growth left, the acknowledgment is unnecessary and its byte delta was
no longer true, so the shared ack fragment is restored byte-identical to base.

The docs for this change were entirely Reference — the flag and the warning
codes — with the task-oriented quadrant empty. Adds the page that answers the
question an operator actually has when the advisory fires: what the two codes
mean, that nothing is blocked so there is no failure to hunt for, how to tell
whether the executor over-reached or the plan under-declared, and the three
ways the check legitimately stays silent so an absence of warnings is not
mistaken for proof of conformance.

ffd5370464 fix(#2903): use the command form that actually works in reader-facing docs (#3047)
* fix(#2903): use the command form that actually works in reader-facing docs

Docs told readers to type the colon form, which no runtime registers -- 18 of
19 runtimes use slash-hyphen and the 19th uses shell-var -- so anyone copying an
example got an unrecognized command. Swept 178 occurrences across 53 files,
locale mirrors included so they do not re-diverge from English.

The colon form is a source-authoring token, not a user-facing one: install-time
converters key on it to produce the hyphen form runtimes actually register. So
the sweep is scoped, and three things are deliberately left alone:

- ADRs, which are a historical record; editing their prose falsifies what was
  written at the time.
- The legacy release-notes archive, pending a maintainer decision on whether it
  follows the same historical carve-out. Excluding it keeps a later reversal
  additive rather than a revert.
- Source artifacts under commands, workflows and agents, where the colon form is
  load-bearing. Rewriting those would break the installed-skill guarantee across
  every runtime -- the single largest hazard here.

The plugin namespace form is a real, separate token and survives untouched.

Adds a lint enforcing exactly that boundary, since the correct form genuinely
differs by directory and nothing previously caught the drift.

Also fixes a hardcoded colon form in the capability-matrix generator. The sweep
alone would have left the generated matrix disagreeing with the template that
produces it, so the fix is at the source and the output regenerated.

Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>

* fix(#2903): stop the sweep misquoting source frontmatter

Adversarial review caught three lines where the sweep rewrote a citation of the
literal YAML name: key from a source command file. That key genuinely is the
colon form -- this change's own carve-out logic says source-authoring tokens keep
it -- so the docs ended up misquoting the real files. One of the three is an
acceptance-checklist assertion, which the sweep turned into a false statement.

Restored the three citations to match their sources verbatim, surgically: where a
line carried both a name: citation and a real reader-facing slash command, only
the citation reverted and the command stayed corrected.

The guard needed the same distinction, or it would have flagged the restoration
and reddened the build: a gsd:<cmd> token preceded by name: is a citation of a
source token and is now permitted. The exemption is deliberately narrow -- a bare
gsd:<cmd> anywhere else still fails -- with a test pinning that narrowness.

Also makes the detection case-insensitive. Review found /GSD:next slipped through
silently; no such casing exists in the tree today, so this closes a latent gap
rather than fixing a live one.

Swept the whole tree for further corrupted citations: none beyond the three.

* fix(#2903): retire the stale-next invariant and sweep next like every other command

Maintainer decision on a genuine conflict between two contracts.

Invariant #3054 banned the literal /gsd-next from user-facing docs because it
named a retired workflow-advance command. But commands/gsd/next.md is a live
command -- the state-aware smart-entry launcher -- and this issue requires docs
to use the hyphen form every runtime actually registers. Both could not hold for
this one command, so docs had been sidestepping the ban by keeping the colon
form, which is exactly the defect this issue exists to remove.

FEATURES.md already recorded the reassignment: the hyphen form "is not the
retired workflow-advance command; it is reserved for the state-aware smart-entry
launcher. Workflow advancement remains under /gsd-progress --next." With that
reassignment the invariant's premise is obsolete and the guard now contradicts
the documented command form, so it is retired with a comment recording why
rather than deleted silently.

next is now swept like every other command, and the earlier exemption added to
the new guard is removed so nothing is special-cased.

Four citations of the literal name: frontmatter key stay in colon form, because
the source file really does carry name: gsd:next and a doc quoting it must
reproduce it verbatim. Two of those lines were reworded to say which side is the
frontmatter key and which is the slash command, since they previously conflated
the two.

Verified the retired scan would now genuinely fail against this tree -- the
conflict was real and resolved, not dodged.

publish, version, import, update, remove, turn off — but nothing covering a
change of maintainer for an entry that already exists. Adds
docs/how-to/take-over-a-capability-or-eos.md defining four takeover modes
(consensual handoff, adoption fork, first-party absorption, retirement), the
PR shape each takes, a per-surface snapshot of the inherited user-visible
contract, and an install-continuity checklist.

Also corrects .github/PULL_REQUEST_TEMPLATE/registry-entry.md, which directed
contributors to a 'Registry' Discussions category that does not exist — the
category is named 'EoS Registry' per docs/registries/README.md, and because
'discussion' is a required field the thread must exist before the PR is
opened, so the wrong name stalled contributors at the first required step.

640eaee16e chore(#2930): fragmentize execute-phase.md and prove per-runtime composed emission (#2972)
* feat(#2930): fragmentize plan-phase.md workflow into per-runtime-composed sections

Adds src/workflow-fragments.cts (in-file <!-- gsd:section --> marker
parser/composer, ADR-1671 epic #1671 Phase 3), wires it into
bin/install.js's copyWithPathReplacement emission path, and pilots the
marker grammar on gsd-core/workflows/plan-phase.md.

Bookkeeping ripple for the new src/*.cts module: .gitignore,
eslint.config.mjs, docs/INVENTORY.md + docs/INVENTORY-MANIFEST.json,
and a CONTEXT.md glossary entry. Amends ADR-1671 with open questions 1
and 2 resolutions and records the closed when= applicability grammar.
Adds docs/reference/workflow-fragments.md and an ARCHITECTURE.md
section documenting the marker authoring model.

* fix(#2930): put allow-test-rule issue ref on the same line as the marker

lint-allow-test-rule-refs.cjs requires the #NNN issue reference on the
same source line as `allow-test-rule:`; it was one line below and read
as an unreferenced novel exemption.

* docs(#2930): link the orphaned gate-predicates reference from the docs index

Found while adding the workflow-fragments reference doc: docs/reference/gate-predicates.md
shipped without an entry in docs/README.md, so it was unreachable from the docs index.
Fixed inline rather than deferred.

* fix(#2930): scope composition to workflows, add typed failure reasons

Review findings from two orthogonal passes:

- Scope composeWorkflow to gsd-core/workflows/ only. It previously ran on
  every .md the installer copied, so a future agent/command/reference doc
  documenting the marker syntax with an unfenced example would have been
  mis-parsed and silently stripped — a lossy drop the phase forbids.
- Add a frozen REASON enum; failures attach a typed .reason and tests assert
  on it instead of matching free-form message text (CONTRIBUTING.md:635-694).
- Derive the property generator's when= values from WHEN_VOCABULARY instead
  of duplicating them (DEFECT.GENERATIVE-FIX).
- Add adversarial parser fixtures: Unicode headings, NUL, U+FFFD, BOM,
  fence-within-fence, tilde and indented fences, lone-CR marker line.
- Document why --mvp is structurally unmarkable: its content is interleaved,
  not sectioned, so the whole-line grammar cannot reach it.

Also fixes two stale tests on this branch, each reproduced on the unmodified
tree before correction.

* fix(#2930): retarget the pilot from plan-phase to execute-phase

The full remote matrix went red on both Linux lanes. Root cause was ours:
tests/phase6-capstone-conformance.test.cjs holds a PRE_PHASE6 ceiling of
94519 bytes for plan-phase.md, asserting an ADR-857 Phase-6 completion
property. That is a third size gate beyond the tier caps and the
differential ratchet, and it left plan-phase.md just 36 bytes of headroom
rather than the 3821 computed from the XL cap. The 330 marker bytes
overran it by 294.

Raising the ceiling is not an option: it is a red line certifying another
ADR's completion. plan-phase.md is reverted to byte-identical origin/next
and the pilot moves to execute-phase.md, which has 728 bytes of headroom
under its own ceiling and lands at 93147 with 3 marker pairs.

The vocabulary narrows to the atoms actually used: always, flag:--wave,
state:gap-closure-phase, state:has-prior-phases.

Recorded in the ADR: every branch the epic names lives in plan-phase.md,
which cannot be fragmentized until caps move from source to emitted bytes.
That is direct evidence for the epic's premise and may reorder phases 3-4.

* chore(#2930): backfill changeset PR number (#2972)

* fix(#2930): make the emission install tests portable on Windows

The windows-latest lane went red on three tests in the new install suite;
Linux was green. Both causes were in the test harness, not the module.

Root normalization: the opencode converter always embeds the install root
forward-slashed, but the tests stripped it with the native-separator string
from mkdtemp. On Windows that never matched, so the root leaked through
unstripped — and because the real and stub install roots have different
prefix lengths, that length difference landed directly in the byte-delta
assertion (344 observed vs 275 expected). Normalize both text and root to
one separator form before stripping.

@-ref resolution: the helper stripped only the @~/ and @$HOME/ forms, so a
Windows absolute ref (@C:/Users/...) fell through and was joined onto the
root, producing ...\@C:\Users\... Strip the @ first, then detect
absoluteness from the token's own shape (POSIX, drive-letter, or UNC) with
no platform branching, so every OS takes the same path.

Neither assertion was weakened; the exact-equality byte check is the point
of the test and still holds.

* docs(#2930): document every REASON member and guard the doc/enum parity

Code review found the reference doc's 'Fails closed' list covering 10 of the
11 frozen REASON members — MALFORMED_ATTRIBUTES (parseAttrs rejects malformed
key="value" syntax) had no bullet, and it is distinct from
UNRECOGNIZED_ATTRIBUTE, which is valid syntax with an unknown key.

Two parallel surfaces sharing one constant with nothing asserting they agree is
the DEFECT.GENERATIVE-FIX class, so the same commit adds the parity assertion:
the test derives the enum side from the built module and the doc side by parsing
the reference page, keyed on the reason IDENTIFIER rather than prose so a
reworded bullet does not break it, and reports set differences in both
directions by name.

Proven non-vacuous: removing the MALFORMED_ATTRIBUTES bullet turns the suite
red naming that exact member; restoring it returns 44/44.

49793465d7 docs(#2915): how-to for listing a reviewer lane, and correct the stale listing section (#2917)
* docs(#2904): how-to for listing a reviewer lane in the registry

#2912 shipped the Reviewer Lane Registry, which lands in 1.9.1. Two docs
consequences.

New: docs/how-to/list-your-reviewer-lane.md. A Diataxis how-to for the
publish task -- which of the three catalogs applies (and why a runtime
carrying a reviewer body lists under its primary install shape instead),
opening the required discussion thread BEFORE the PR, the three fields
that reject entries most often (slug grammar differs from id, flags stay
kebab when the slug is snake, install/uninstall must be copy-pasteable),
regenerate-don't-hand-edit, and register-once-then-Releases. Links the
registry README for the field table rather than duplicating it -- the
spec is reference, this is the task flow.

Corrected: ship-a-reviewer-lane.md said "Listing your lane is not wired
yet" and pointed at #2904 as future work. #2906 merged at 11:25Z and
#2912 at 12:11Z, so that section shipped false the moment the registry
landed. Replaced with the publish pointer.

Indexed the new guide and the generated catalog in docs/README.md, and
added the guide to develop-a-capability.md's ecosystem list.

* docs(#2904): caveat credential-bearing configKeys in both the guide and the spec

Isolated security review found the worked entry's `configKeys:
["acme.api_key"]` modelled storing a live credential with no note on
where that value ends up.

Verified: config values are written in plaintext to
.planning/config.json (docs/CONFIGURATION.md:227 -- masking is
display-only, "that file is the security boundary"), and
planning.commit_docs defaults to true (:466). So a credential declared
that way lands in the installing user's git repository unless they have
gitignored .planning/. None of the twelve first-party lanes does this --
they own only review.models.*, host, and prompt-budget keys.

The pattern originates in docs/registries/README.md:221, shipped by
#2912, so the caveat goes on BOTH surfaces rather than only on the copy
that inherited it -- the spec's example is what future authors will read
first.

557d46984e docs(#2782): how-to for declaring a reviewer lane in a capability (#2906)
* docs(#2782): how-to for declaring a reviewer lane in a capability

ADR-2782 shipped the Reviewer Lane capability surface in 1.9.0, but the
only documentation was reference (capability-manifest.md) and rationale
(ADR-2782). A capability author had no task-oriented path from "I have a
review CLI" to "/gsd:review invokes it".

Adds docs/how-to/ship-a-reviewer-lane.md: role selection, the spawn and
openai-http worked examples, federated config ownership, the
review-lane query surface as the verification step, what the install
disclosure and egress-host re-verification mean for the author, and the
data-only boundary with the two named CLIs that do not fit today.

Also corrects the manifest reference's `invoke` row, which understated
three enums against the shipped validator: promptChannel omitted `argv`,
effortChannel omitted `env`, and the openai-http sub-shape plus the
required-with-file-arg `outputArg` were undocumented entirely.

* docs(#2782): correct four claims found by the two orthogonal reviews

Security review (1 major, 2 minor):
- "a reserved slug" implied the gsd-/anthropic- namespace rule, which
  guards the capability id, not reviewer.slug. The slug guard is
  isReservedName (__proto__/constructor/prototype), a prototype-pollution
  barrier. Both rules are now stated and kept apart.
- Added ADR-2782 D5's own caveat verbatim: disclosure and host pinning
  make the channel visible, pinned and revocable, not safe, and
  consent-at-install is a weaker gate for a standing egress channel than
  for a hook.
- Named integrity/SHA pinning and engines.gsd as the controls that make
  the disclosure tamper-evident and the version range enforceable.

Correctness review (1 major, 1 minor):
- Claimed a name collision is "a hard failure at install". It is not.
  installCapability never runs validateCrossCapability; the check runs in
  loadRegistry, and a colliding overlay is dropped from acceptedMap with a
  warning while the install reports success. Documented as the quiet
  failure mode it is, with the symptom to look for.
- The feature-only field list omitted hooks and activationKey, both of
  which FEATURE_FIELDS_FORBIDDEN_ON_REVIEWER rejects.

Both worked examples re-validated against validateCapability() -> [].

* docs(#2782): restore Kimi Code to the cross-AI reviewer list

set-up-cross-ai-review.md named eleven reviewers; twelve lanes ship. The
kimi-code lane (added by #2718, declared as manifest data by #2798) was
never added here — the same roster-drift class as #2781, which #2800's
parity gate covers for COMMANDS.md and FEATURES.md but not for how-to
prose.

Also points readers at the declared-lane model rather than a static list:
the roster is now generated, a capability can ship its own lane, and
`gsd-tools review-lane sections` answers "what do I actually have".

Verified against the twelve declared bodies in capabilities/*/capability.json.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* docs(#2782): use the invocation form the runtime descriptors actually declare

The new guide used /gsd:review. Nothing in this repo produces that form.

- capability-validator VALID_COMMAND_STYLES is {slash-hyphen, shell-var};
  there is no colon/namespaced style in the vocabulary at all.
- 18 of 19 runtime capabilities declare commandStyle "slash-hyphen",
  claude included; codex is "shell-var". Every artifactLayout prefix is
  "gsd-".
- A plain-file command install never namespaces, so .claude/commands/
  gsd-review.md is typed /gsd-review.
- The Claude Code plugin surface would namespace on plugin.json "name",
  which is "gsd-core" -- so the plugin form would be /gsd-core:review.
  The commands/gsd/ subdirectory is cosmetic and contributes nothing to
  the invoked name.

So /gsd:review is neither the installed form nor the plugin form. Uses
/gsd-review, matching set-up-cross-ai-review.md and the 18 descriptors.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* docs(#2782): state that lane listing is not wired yet

The guide could describe authoring, validating, and installing a lane but
implied a publishing path that does not exist. Neither discoverability
catalog can accept one: a Community Capability Registry entry requires a
non-empty loopExtensionPoints plus hookKinds, and a role:"reviewer"
capability is forbidden from declaring steps/contributions/gates, so both
fields are unsatisfiable rather than merely unset. The EoS Registry is
ADR-1239 host integrations, which a lane is not.

The registry schema predates the reviewer role by 17 days (#2182 Jul 11,
ADR-2782 Jul 28) and registry-schema.cjs has zero occurrences of
"reviewer". Tracked for a 1.9.x point release by #2904.

Says so explicitly, and tells authors NOT to file a loop extension point
they do not use to get past validation -- a schema satisfiable only by
lying is one that will be lied to.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* chore(#2782): backfill PR number and fix the changeset invocation form

pr: 0 -> 2906.

Also corrects /gsd:review -> /gsd-review in the fragment body. The
fragment renders into CHANGELOG.md, which is a reader-facing docs surface
and is never passed through the install-time converter -- so the colon
form would ship the #2903 drift into a permanent release artifact.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>

* docs(#2907): propagate the invocation-form fix and restore lane order

Two findings from an isolated review of the post-review delta.

- docs/README.md and develop-a-capability.md still said /gsd:review in
  the cross-links added for the new guide. The form was corrected in the
  guide itself but not in the two entries pointing at it, leaving three
  docs making the same claim in two different forms.
- set-up-cross-ai-review.md inserted Kimi Code between Antigravity and
  Ollama. REVIEWER_LANES is ordered by write_reviews order and kimi-code
  is 12th, appended after llama_cpp; the prose list mirrored declaration
  order before this change and now does again.

c4237df8e6 docs(#2276): 1.7.0 release documentation — what's-new, EoS explanation, feature index (#2282)
Add a curated 1.7.0 release-highlights page (docs/whats-new-1.7.0.md) and a
conceptual Embeddable Orchestration System (EoS) explanation
(docs/explanation/embeddable-orchestration-system.md), extend docs/FEATURES.md
with a v1.7.0 feature section, and wire both new docs into the docs index
(docs/README.md) and the root README.

Covers the release's marquee changes: the ADR-1239 Host-Integration Interface /
EoS (Embeddable Orchestration System) runtime expansion, the Capability + EoS
discoverability registries, the gsd-mcp-server companion, model-catalog advances
(GPT-5.6, (1M) badge), statusline enhancements, the compact GSD-state format,
plus a themed summary of the 100 fixes and 4 security hardenings.

Also corrects a stale CONTEXT.md glossary entry: the Capability Registry Overlay
now documents the #2009 fail-open behavior for a load-failed gate-declaring
capability (previously described as fail-closed).

American house style; no parity-gated reference docs hand-edited.

46f8d3814c fix(#2009): load-failed capability gates fail open with a loud warning
Previously a capability that failed to LOAD (e.g. incompatible engines.gsd) but
declared a gate-kind loop hook caused the loop resolver to inject a BLOCKING
synthetic gate (blocking:true, onError:halt) at every declared point, halting
every ship:pre / verify:post project-wide over an unrelated load error, with no
remediation surfaced.

Per maintainer decision (#2009) it now fails OPEN: no gate is injected (the loop
proceeds; --active-cap correctly reports the failed cap inactive) and a loud
warning is emitted — to stderr (the channel host workflows/agents actually see)
and in the envelope 'warnings' array — naming the load reason and the exact
'gsd capability remove <id>' remediation. The loader still records blockedGates;
only the consequence changes from block to warn.

Security (review): capId and reason originate from a third-party manifest /
directory name. capId is validated against the canonical kebab-case id shape
before it is placed in the runnable remediation command (withheld otherwise);
reason is stripped of control chars and backticks. This closes an argument/
prompt-injection vector in the surfaced message.

Docs updated to the fail-open-warning posture (ARCHITECTURE, INVENTORY,
CONFIGURATION, README, capability-overlay-model). Also removes a dead 'before'
import surfaced by lint in the issue-2045 test.

Code-review hardening: treat NaN/Infinity maxDepth as missing (fail-closed, +warning); reset nested/background when namedDispatch collapses to false (struct consistency); SAFE_DEFAULTS dispatch floor to read-only; warn on non-finite protocolVersion; symmetric undocumented warnings for dispatch fields. Pure module — no consumers; behaviour fail-closed throughout.

ADR-1244 Phase 6. Adds the capability matrix reference, generated FROM the committed registry so it
can never drift from the actual capability set:
- scripts/gen-capability-matrix.cjs (--write / --check); --check is a CI drift guard.
- tests/capability-matrix-sync.test.cjs (4 tests): drift guard, buildMatrix==committed, every cap
  present, no placeholders.
- docs/reference/capability-matrix.md regenerated from the registry (release-stable: shows engines.gsd,
  omits the lockstep per-cap version that would churn the file every release).
- Consolidated the duplicate trust-model doc: deleted docs/explanation/the-capability-trust-model.md,
  merged its content into capability-trust-model.md, redirected ~10 references; no stale links remain.
- Diataxis verification (now that gsd capability is a real command): corrected the matrix's third-party

- Replace phantom agent slugs (gsd-checker, gsd-researcher, gsd-advisor, gsd-synthesizer) in CONFIGURATION.md Supported Agent Types with real agents/gsd-*.md examples (gsd-plan-checker, gsd-phase-researcher, gsd-code-reviewer, gsd-ui-auditor, gsd-research-synthesizer)
- Add byte-identical mixed-block regression test: one path-resolvable global skill + one plugin-namespaced skill on claude runtime → asserts r.ir.block === single-section interleaved block, no secondary header

Adds the spec-time prohibition probe (spec-phase Step 5.6) — the second adapter of the probe-core resolution model. Surfaces unwritten must-NOT constraints as negative SPEC acceptance criteria with test/judgment verification tiers; fail-closed at verify time. Per ADR-550. Closes #644.

* feat(#1213): Capability State Writer — write-side inverse of the resolver

Adds src/capability-writer.cts (setCapabilityState + cmdCapabilitySet) and the
`gsd-tools capability set` subcommand: the write-side inverse of the capability
resolver (ADR-1213). One desired capability state projects onto the substrates —
`enabled` drives the runtime surface (canonical on/off), `gates` drive federated
config keys (hook granularity), install profile is a read-only floor — then
re-resolves and reports divergence (assert-and-report), so "off means off" holds
as a write-time invariant. Adds batched setConfigValues; routes gsd:settings
capability hook-gates through the writer. Docs: CLI-TOOLS reference, how-to,
ADR-1213, CONTEXT.md term.

3e836fef0d feat(spec-phase): spec-completeness edge-probe (#550) (#584)
* feat(spec-phase): spec-completeness edge-probe (#550) — relocated to gsd-core/

Rebased onto current next and relocated the whole feature from get-shit-done/ to
gsd-core/ per #615 (trek-e re-review #4, option 1). The artifact now builds to
gsd-core/bin/lib/edge-probe.cjs; all hard-coded path strings (tests, workflow
@-refs, run-tests.cjs sentinel, eslint ADR-457 ignore, .gitignore) updated.
Content conflicts in .gitignore / eslint.config.mjs / run-tests.cjs resolved.

Feature: Step 5.5 edge-completeness probe walks each SPEC requirement against a
closed 8-category edge taxonomy, proposes applicable candidate edges, and resolves
each (covered/dismissed/backstop/unresolved). covered/backstop criteria are lifted
by plan-phase into must_haves.truths, extending the goal-backward verifier's reach
to boundary edges no requirement was written for. Engine authored as strict TS
(src/edge-probe.cts, ADR-457), compiled to the gitignored gsd-core/bin/lib/edge-probe.cjs.

Folds in every prior review round on PR #584:
- RR-01..03: plan-phase resolves the phase *-SPEC.md and injects {SPEC_PATH} into the
  planner; must_haves<->Edge-Coverage quality_gate; held-out planner-contract test.
- RR-04/11: Step 5.5 invokes the compiled engine at runtime (npm --prefix-pinned,
  source-checkout-gated build fallback) instead of LLM re-derivation; the engine
  capture is exit-checked and the report JSON-validated before use (fail closed).
- RR-05..10: all six fixtures embedded + count-equality; backstop/covered require a
  resolution; Array.isArray(shapes); duplicate-resolution rejection; CLI JSON exit(2);
  per-artifact build sentinel.
- Authored-shape validation: invalid (non-empty) shapes fail closed (VALID_SHAPES).
- Adversarial-review hardening: orphan/typo resolution rejection, requirement input
  validation (id/text/shapes, duplicate id, non-array), zero-applicable guard.

Full suite 0 failures; npm run lint 0 errors; edge-probe suite 72/72.

* test(#550): RED — status×verification re-cut + probe-core engine specs

Re-cut the edge-probe resolution model onto two orthogonal axes per
ADR-550 Decision 7 (trek-e #644 comments 2026-06-03 14:36 + 14:44):

  status: resolved | dismissed | unresolved (lifecycle, shared)
  verification: explicit | backstop | null (only when resolved)

- tests/probe-core.test.cjs (new): behavioral specs for the generic engine
  to be extracted — validateResolution(r, validators), validateRequirement,
  analyzeCoverage(items, resolutions?, validators), byVerification rollup,
  runProbeCli I/O scaffold (injected-io unit tests).
- tests/edge-probe.test.cjs: covered→{resolved,explicit}, backstop→
  {resolved,backstop}; coverage gains byVerification.{explicit,backstop};
  proposeEdges items gain verification:null.
- 6 fixtures re-genned + re-embedded in edge-probe.md; coverage.resolved
  COUNT preserved on every fixture (closed set = resolved+dismissed; doc
  line: 'adjacency=covered + ordering=dismissed' -> 2). edge-probe.md prose
  rewritten to the two-axis model.

Fails as expected: probe-core.cjs has no source yet; edge-probe still
emits the old covered/backstop enum (27/61 edge specs red).

* feat(#550): extract probe-core seam + refactor edge-probe onto it (ADR-550 D7)

Extract the generic resolution model into src/probe-core.cts (the shared
seam the prohibition probe #644 is born on) and refactor edge-probe.cts
into its first adapter.

probe-core owns (probe-agnostic):
- the status×verification re-cut: status: resolved|dismissed|unresolved ×
  verification: <probe-defined>|null
- validateResolution(r, validators) / validateRequirement (generic id+text)
- analyzeCoverage(items, resolutions?, validators) over ALREADY-PROPOSED
  items[] (core never assumes propose is deterministic — edge resolves via
  LLM, #644 proposes via LLM), with merge / dup-reject / orphan-reject
- byVerification rollup; coverage.resolved = closed set (resolved+dismissed),
  count-preserved from the pre-re-cut engine
- runProbeCli I/O scaffold (injected io; one bin per probe)
- hybrid typing: generic params + injected runtime validators
  {categories, verification, requiredFieldsByVerification} (ADR-550 #5)

edge-probe keeps ONLY the edge cluster: Shape/SHAPE_CUES/VALID_SHAPES/
classifyShape/TAXONOMY/applicableCategories/proposeEdges + EDGE_VALIDATORS
{explicit,backstop}; delegates merge/rollup/CLI to probe-core. Every shipped
#584 guarantee preserved (fail-closed shapes, orphan/dup rejection, input
validation, CLI exit 2). 104/104 edge+probe-core+docs+contract specs green.

* chore(#550): register probe-core.cjs artifact in ledgers + inventory

New gitignored build artifact gsd-core/bin/lib/probe-core.cjs (compiled
from src/probe-core.cts) needs registering in every artifact ledger:

- .gitignore + eslint.config.mjs ADR-457 ignore: lint the .cts source,
  never the emitted .cjs.
- scripts/run-tests.cjs per-artifact build sentinel: build if probe-core.cjs
  is missing on a clean checkout.
- docs/INVENTORY.md: CLI Modules 83 -> 84, new probe-core.cjs row, and the
  edge-probe.cjs row updated to reflect it is now the first probe-core adapter.
- docs/INVENTORY-MANIFEST.json: regenerated (gen-inventory-manifest.cjs --write).

probe-core.test.cjs is a single test file (under the 2-file cap), so no
lint-test-file-count allowlist entry is needed.

* docs(adr-550): spec-phase probe pattern + prohibition contract [Accepted]

trek-e's final ADR-550 body, verbatim (open-gsd/gsd-core#644 comment
2026-06-03T15:23Z), Accepted by both maintainer and #550 author. Lands on
PR #584 alongside the probe-core extraction (Decision 7) it governs, so the
contract and its first implementation arrive together.

Decisions: probe packaging (3 layers); recall->precision protocol;
prohibition home = SPEC acceptance criteria + optional must_haves.prohibitions:
(truths untouched, no polarity); tiered verification test|judgment
(judgment = mode-dependent soft-gate-with-flags, never silent pass / never
hard-halt); CI tests the contract not the classifier; secure-phase ownership
seam; and Decision 7 — probe-core seam + status×verification re-cut (7a-7e),
which this PR implements.

* feat(#550): fail-closed probe-core across full status×verification + runProbeCli structural guard

Re-review #5 (trek-e) seam-hardening on the generic probe-core contract #644 inherits:

- validateResolution now enforces the 'verification is null unless resolved'
  invariant for EVERY status (not just resolved): a dismissed/unresolved
  resolution carrying a verification tier is rejected instead of merging verbatim.
- An unresolved resolution carrying a resolution/reason payload is rejected
  (was silently dropped into the unresolved count).
- runProbeCli structurally validates the report an adapter returns before writing
  it (was: any malformed object stringified as green output) — fails closed → exit 2.
- coverage.resolved kept count-preserved (closed set) per the blessed migration
  contract; a new test locks that an all-dismissed run is NOT affirmatively covered
  (byVerification is the honest gate).

Tests: probe-core 37/37; full edge-probe suite 113/113; full suite 1816/1816; lint 0.

* docs(adr-550): annotate Decision 5 #584/#644 scope + correct 7a coverage.resolved semantics

Re-review #5 (trek-e) clarity edits:

- Decision 5: annotate that only contract item (a) ships on #584 (the edge
  adapter's parse+validate test); (b)–(d) are #644 scope, matching Consequences.
- Decision 7a: correct the 'coverage.resolved is preserved (status === resolved)'
  parenthetical — the blessed/implemented semantics are count-preserved = the
  CLOSED set (resolved + dismissed = applicable − unresolved), with byVerification
  carrying the per-tier resolved-status breakdown. The old parenthetical
  contradicted the shipped count.

* test(#550): cover runProbeCli structural-guard numeric-count branch

Second-pass coverage audit found the 'coverage object present but counts
non-numeric' branch of isValidReport (built probe-core.cjs:60-61) unexercised —
the {nope:true} malformed test fails earlier at the items[] check. Add a report
with well-formed items[] + a coverage object carrying non-numeric counts so the
numeric branch is hit. No source change; closes the line gap.

* fix(#550): reject edge requirement with missing/empty text when no shapes override (M2)

The edge adapter's `text` is the classification signal and a required field, but
core `validateRequirement` left it optional, so a `{ id }` requirement classified to
zero shapes -> zero edges -> was silently DROPPED from coverage with no signal -- the
exact fail-open this feature exists to eliminate. Reject missing/empty text unless an
authored `shapes` override (incl. `[]`) opts out of prose classification.

* fix(#550): validate verbatim items in analyzeCoverage shared seam (m1)

A proposed item with no matching author resolution is rolled up VERBATIM, but its own
status/fields were never validated -- an item carrying an out-of-enum status (the dropped
"covered") or `dismissed` without a reason would be counted closed. The edge adapter only
proposes `unresolved` items, but the prohibition adapter (#644) proposes LLM-generated
items that arrive populated. An Item is structurally a superset of a Resolution, so reuse
validateResolution to fail closed. ADR-550 Decision 5 hardens this shared seam.

* fix(#550): move edge-coverage lift instruction to runtime planner surface (M1)

templates/planner-subagent-prompt.md is loaded by nothing at runtime (no @-import in
agents/gsd-planner.md; plan-phase.md spawns the planner from its own inline
<planning_context>), so the precise covered/backstop -> must_haves.truths lift instruction
this PR added there never reached the planner -- and the RR-02 contract test asserted it in
that dead file, giving false green. Move the instruction into plan-phase.md's runtime
<downstream_consumer> block (where the rest of the wire already lives), revert the dead-template
edit, and retarget RR-02 to the loaded surface with a guard against re-orphaning.

* test(#550): lock machine<->SPEC vocabulary mapping against drift (m2)

The machine contract uses orthogonal status x verification; the SPEC table renders a flat
covered/dismissed/backstop/unresolved. The migration map (ADR-550 Decision 7a) was prose-only
with no test, so the layers could silently drift. The SPEC table is LLM-rendered (no JS
renderer to round-trip), so pin the canonical bijection as code AND ground it in every doc
surface that renders the vocabulary (ADR migration clause, spec.md legend, reference mapping
table) -- a rename or remap now fails the suite.

* docs(#550): add how-to for resolving edge-coverage findings (B1)

Feature shipped reference coverage (FEATURES.md, COMMANDS.md, references/edge-probe.md) but
no how-to -- reference-only does not satisfy the Diataxis docs standard for a user-facing
capability. Add a single-mode how-to (imperative, goal-directed) walking each resolution
state (specify/dismiss/backstop/defer), the soft gate, and --auto, with taxonomy/concepts
linked out to the reference. Register it in the docs/how-to index.

* docs(#550): add Probe Core + Edge Probe glossary entries to CONTEXT.md (N1)

trek-e re-review #7 N1 (Major): adding probe-core/edge-probe as src/*.cts-derived
seam modules (ADR-550 Decision 7) requires CONTEXT.md Domain-terms glossary entries
per the maintainer-enforced new-seam gate. Adds '### Probe Core Module' and
'### Edge Probe Module' with exports, generated source paths, and the ADR-550 seam
contract, placed beside the Research Module feature-seam entries.

* test(#550): add fast-check property suite for probe-core analyzeCoverage (N2)

trek-e re-review #7 N2 (RULESET.TESTS.property-based-testing): analyzeCoverage is a
transformation/rollup module, the class the property-testing predicate covers, and
fast-check is already a dependency with an established *.property.test.cjs pattern.
Adds 5 properties over the algebraic invariants: closed-set identity
(applicable === resolved + unresolved), byVerification sums ≤ resolved, per-tier
recount + resolved-status-only counting, rollup determinism, and stable orphan
rejection. 200 runs/seed 42 via helpers/fast-check-setup.cjs.

* test(#550): align allow-test-rule tokens to canonical runtime-contract-is-the-product (N3)

trek-e re-review #7 N3 (Nit): the // allow-test-rule: tokens (source-text-is-the-product,
docs-parity) differed from CONTEXT.md's canonical exemption category
'runtime-contract-is-the-product' (RULESET.TESTS.no-source-grep.exemption, CONTEXT.md:240).
All three tests assert deployed runtime-contract surfaces (spec-phase.md Step 5.5, the
plan-phase.md planner prompt, the rendered reference/SPEC/ADR vocabulary), so the canonical
category fits; each now carries a one-line justification per the ruleset format. Free-text
reason — lint behavior unchanged.

* test(#550): re-baseline plan-phase + spec-phase byte sizes for edge-probe

Rebased onto next (e4f0910d), which replaced the line-based tier-max
size ratchet (#597) with the byte-based per-file baseline guard (#1074).
The edge-probe feature legitimately grows two workflows:

  - spec-phase.md  15131 -> 23094 (+7963): Step 5.5 Edge-Completeness Probe
  - plan-phase.md  93135 -> 94253 (+1118): covered/backstop edge lift into
    must_haves.truths (the live <downstream_consumer> block)

Both remain under their tier hard caps (plan-phase XL 98304, ~4KB
headroom; spec-phase DEFAULT 40960). Growth is real inline workflow
content the feature requires at that step — not eager @-import proxy
gaming. Drops the obsolete line-based XL_BUDGET 93000->94000 bump
(superseded by the byte baseline) via rebase.

* chore(#550): reconcile INVENTORY headline counts after rebase onto next

Rebase onto current next dropped the prior reconcile commit (stale counts
refs 68 / CLI 107). Current next + the edge-probe additions yield:
  - References (68 -> 69 shipped): + gsd-core/references/edge-probe.md
  - CLI Modules (107 -> 109 shipped): + edge-probe.cjs + probe-core.cjs

Rows for all three already present; only the headline counts were stale.
Caught by tests/inventory-counts.test.cjs (CI ubuntu-24 leg).

69f6efebf1 docs(#743): address Kimi review — runtime count + support boundary
Address remaining review feedback on PR #743:

- Bump supported-runtime count 15 -> 16 across user-facing READMEs
  (docs/README.md + ja-JP/zh-CN translations) now that Kimi is added.
- Add an explicit support-boundary note in the Kimi CLI install section:
  the kimi --agent-file custom-agent contract targets legacy/Python
  kimi-cli; newer npm Kimi Code (@moonshot-ai/kimi-code) rejects
  --agent-file and consumes the same /skill:gsd-* skills via --skills-dir.
- Mirror the boundary in the changeset so release notes are unambiguous.

Historical ADR/changeset/INVENTORY references to '15 runtimes' are left
intact as point-in-time records.

cf8bd3cd5e fix(#683): auto-degrade phase execution to sequential on worktree base mismatch (#749)
* fix(#683): auto-degrade phase execution to sequential on worktree base mismatch

Claude Code forks worktree-isolated executors off the repository default
branch (origin/HEAD), not the orchestrator's HEAD. Running /gsd-execute-phase
on a branch diverged from the default (unmerged milestone/feature branch) left
every executor without the phase's plan files and tripped the
worktree-branch-check guard with `exit 42` — 100% reproducible, all OSes.

- New module src/worktree-base-ref.cts: HEAD-vs-fork-base drift detection
  (origin/HEAD with symbolic-ref fallback) and no-clobber worktree.baseRef
  management, exposed as `worktree base-check` / `worktree set-baseref`.
- execute-phase.md: pre-dispatch, for Claude Code with worktrees enabled,
  auto-degrades the run to sequential on the main tree when a base mismatch
  is detected, recommending worktree.baseRef:"head". The exit-42 guard stays
  as a backstop.
- Installer: fresh local Claude installs set worktree.baseRef:"head" in
  .claude/settings.local.json (no-clobber, respecting an explicit shared
  settings.json value); upgrades print an opt-in notice pointing at
  `gsd-tools worktree set-baseref`.
- Docs: how-to guide, CLI/config reference, planning-config cross-ref.

Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

* fix(#683): auto-apply worktree.baseRef on upgrade; gate fresh+upgrade on use_worktrees

Per maintainer direction: on a local Claude Code UPGRADE, set
worktree.baseRef:"head" automatically (no opt-in notice) when the project's
workflow.use_worktrees is enabled, instead of merely printing a remediation
notice. For consistency the FRESH path is now gated the same way: both paths
compute worktrees-enabled once (bounded walk-up read of .planning/config.json,
default enabled unless workflow.use_worktrees === false) and apply the
no-clobber baseRef only when enabled — never overwriting an explicit value in
settings.local.json or a shared settings.json. gsd-tools worktree set-baseref
remains for manual use. Docs + changeset updated; tests hardened (file-exists
assertions, fresh+disabled case, upgrade idempotency).

* fix(#683): measure workflow byte-budget on LF, fixing Windows-only CI failure

The workflow-size-budget test failed only on Windows: git checks out the .md
files as CRLF (no eol=lf in .gitattributes) and byteCount used
fs.statSync().size (raw on-disk bytes), counting an extra \r per line. That
inflated execute-phase.md — the XL high-water-mark file pinned near its ceiling
by the tighten-only ratchet — from 88492 LF bytes to ~90245 on Windows, over
the 90000 XL ceiling, while passing on the LF-checkout Mac/Linux runners.

The ceilings are explicitly "calibrated against raw `wc -c`" on an LF checkout,
so the measurement should be LF-based on every platform. byteCount now reads the
file and counts Buffer.byteLength after stripping CR, making the budget
platform-independent (a no-op on LF checkouts; verified statSync === normalized
for all 88 workflow files). No ceilings changed. Added a regression test
asserting CRLF and LF content of the same file count identically.

* fix(#683): make worktree-base-ref test path mocks Windows-safe (path.join)

tests/worktree-base-ref.test.cjs keyed its injected readFile/writeFile mocks
(and a few expected `file` values) with forward-slash template literals like
`${claudeDir}/settings.local.json`. The module composes those paths with
path.join(), which emits backslashes on Windows, so the mock keys never matched
the module's lookup → readFile returned null → resolveEffectiveBaseRef /
cmdWorktreeBaseCheck / cmdWorktreeSetBaseRef (and the JSONC variants) failed on
the Windows full-test runner only (they passed on Mac/Linux, and the install
tests passed because they use the real filesystem). The module is correct;
only the test fixtures hardcoded '/'.

All mock keys and path assertions now use path.join(base, ...) mirroring the
module, so they match on every platform (no-op on POSIX). 19 path references
across 16 lines.

91f5bd7cb1 feat(#607): rebuild get-shit-done-cc → gsd-core migration (per-package cache + installer auto-cleanup + --dry-run) (#611)
* feat(#607): rebuild get-shit-done-cc → gsd-core migration

Leftover get-shit-done-cc installs poisoned the shared update cache,
causing a permanent false "update available". Rebuild the migration so a
stale old install is both harmless and actively removed.

- Per-package update cache filename (gsd-update-check-<slug>.json) in the
  shared ~/.cache/gsd dir, single-sourced via package-identity; writers
  stamp package_name and readers reject foreign/absent lineage. Multi-
  runtime visibility preserved (same shared dir + filename across runtimes).
- New get-shit-done/bin/lib/legacy-cleanup.cjs seam: detects code-file
  references to the old package + the legacy fixed-name cache across home
  runtime dirs; installer auto-cleans on every install; --dry-run previews
  and mutates nothing. User hooks and dev-preferences are never touched.
- update.md cache-clear globs gsd-update-check*.json across ALL supported
  runtimes (adds cursor/windsurf/augment/trae/qwen/hermes/codebuddy/cline).
- Fix worker MODULE_NOT_FOUND post-install (ship managed-hooks-registry.cjs
  + degrade gracefully) so the per-package cache is always written.
- Diataxis how-to: docs/cleanup-get-shit-done-cc.md.

beb7e05254 docs(3635): add v1.42.3 release notes and link from docs index (#3636)
New docs/RELEASE-v1.42.3.md follows v1.42.1's section layout
(hotfix-shaped: Fixed only, no Added/Changed). Headline is Codex CLI
0.130.0 install routability — after install, $gsd-* skills now
resolve under Codex 0.130.0+ where the previous build left zero
routable entrypoints. Twelve user-visible fix categories covered:

  - Codex CLI 0.130.0 install materializes routable skills (#3562, #3582)
  - Runtime-aware slash formatter (#3584)
  - Argv-based subprocess for check.ship-ready (#3587)
  - phase_status field + /gsd:plan-phase closed-phase gate (#3569)
  - Canonical [features].hooks for Codex configs (#3566)
  - SDK bridge loads via public package export (#3567)
  - W006/W007 skip archived and future phases (#3559, #3560)
  - Ultraplan gates on Claude Code markers (#3561)
  - Padded phase IDs match unpadded ROADMAP prose (#3537)
  - Prompt-user migrations resolve in non-TTY runs (#3541)
  - Executor agents forbidden from git stash (#3542)
  - Configuration manifests load from installed payload (#3571)

docs/README.md updates:
  - Documentation Index gets a row for v1.42.3 above v1.42.1
  - Quick Links "What's new" lists v1.42.3 first, then v1.42.1

Codex CLI 0.130.0 minimum-version note added to the Installing
section so operators upgrading to 1.42.3 know the Codex dependency.

2d32ad82be fix(plan-phase): remove agent: directive that caused OpenCode subagent dispatch (#3156) (#3206)
* feat(roadmap): parse **Mode:** field on phase sections

Adds a 'mode' field to roadmap.get-phase and roadmap.analyze outputs.
Recognizes '**Mode:** mvp' lines in phase sections; lowercased + trimmed.
Forward-compat: unrecognized values preserved verbatim, no enum check.

Foundation for --mvp flag in plan-phase (PRD: vertical-mvp-slice).

* feat(plan-phase): parse --mvp flag and resolve MVP_MODE

Resolution order: CLI flag → ROADMAP **Mode:** field → workflow.mvp_mode
config → false. Walking Skeleton gate fires for new-project Phase 1.
Wires MVP_MODE + WALKING_SKELETON into gsd-planner subagent prompt.

Per PRD vertical-mvp-slice Phase 1 (Q1, Q2, Q4).

* docs(planner): add vertical-slice planning reference

New reference loaded by gsd-planner when MVP_MODE=true. Defines slice
ordering, Walking Skeleton rules, and anti-patterns. Referenced from
plan-phase workflow MVP_MODE wiring.

* docs(planner): add SKELETON.md template

Template emitted by gsd-planner under WALKING_SKELETON=true. Captures
architectural decisions and out-of-scope list for new-project Phase 1.

* chore(inventory): register new planner references

Added planner-mvp-mode.md and skeleton-template.md to INVENTORY.md and
INVENTORY-MANIFEST.json. References now: 53.

* feat(gsd-planner): add MVP Mode Detection section

Mode-switched branch in the existing planner agent (per Q4: single agent).
Vertical-slice decomposition rules, Walking Skeleton handling, and
TDD-mode compatibility. Heavy guidance lives in references/planner-mvp-mode.md.

* test(plan-phase): add --mvp resolution-chain integration cases

Validates roadmap.get-phase --pick mode and confirms workflow.mvp_mode
default is unset in fresh projects.

* docs(changelog): announce --mvp vertical-slice planning (#2826)

* feat(mvp-phase): add /gsd mvp-phase slash command

Standalone command for vertical MVP planning. Frontmatter only;
heavyweight workflow at get-shit-done/workflows/mvp-phase.md follows
in next commit. Mirrors discuss-phase/edit-phase command shape.

* docs(planner): add user-story-template reference

Defines the canonical 'As a / I want to / So that' format and the
ROADMAP.md / PLAN.md emit rules. Used by mvp-phase workflow and
gsd-planner agent under MVP_MODE.

* docs(planner): add SPIDR splitting reference

Defines size signals, the five SPIDR axes (Spike/Paths/Interfaces/Data/Rules),
the interactive workflow, and anti-patterns. Per PRD Q3 decision: full
interactive flow, not lightweight check. Used by mvp-phase workflow.

* fix(mvp-phase): trim description to fit 100-char budget

* feat(mvp-phase): add mvp-phase workflow

Standalone workflow: phase validation -> user story prompts (As a / I want to /
So that) -> SPIDR splitting check -> ROADMAP write (Mode + Goal) -> delegation
to plan-phase. Per PRD Phase 2 (Q3 full SPIDR; Phase-2-A/B/C/D decisions).

Plan-phase auto-detects MVP via Phase 1's resolution chain, so no flags
are needed when delegating.

* feat(gsd-planner): emit user-story header in PLAN.md under MVP mode

Extends the MVP Mode Detection section (added in Phase 1) so the planner
sources the user story from ROADMAP **Goal:** and emits the bolded
**As a** / **I want to** / **so that** form as the first content under
the phase header in PLAN.md. References user-story-template.md.

* test(mvp-phase): integration smoke test for ROADMAP mutation

Validates roadmap.get-phase output after a workflow-spec'd ROADMAP write:
mode=mvp and goal=full user story. Catches schema drift between workflow
emit and parser expectation. Includes a long-story case (>120 chars) to
confirm SPIDR-rejected stories still parse correctly.

* chore(inventory): register mvp-phase command + 2 new references

Adds /gsd mvp-phase to commands list, mvp-phase workflow to workflows list,
and user-story-template.md + spidr-splitting.md to references. References
count: 53 -> 55.

* docs(changelog): announce /gsd mvp-phase command (#2826)

* fix(mvp-phase): add TEXT_MODE plain-text fallback for non-Claude runtimes (#2012)

* docs(executor): add MVP+TDD gate reference

Defines the runtime gate semantics for execute-phase when both
MVP_MODE and TDD_MODE are true: pre-task verification of failing-test
commit, end-of-phase review escalation from advisory to blocking,
behavior-adding task definition. Loaded conditionally by
execute-phase workflow and gsd-executor agent.

* feat(execute-phase): MVP+TDD runtime gate + blocking review

Resolves MVP_MODE in Step 1 (CLI flag -> roadmap mode -> config -> false).
Adds per-task gate that halts before behavior-adding tasks run if no
failing-test commit exists for the plan. Escalates end-of-phase TDD
review from advisory to blocking when both MVP_MODE and TDD_MODE active.

Also updates INVENTORY-MANIFEST.json to register execute-mvp-tdd.md
(added by Task 1) so manifest-sync tests pass.

Per PRD vertical-mvp-slice Phase 3a (decisions Phase-3-A, Phase-3-Split).

* feat(gsd-executor): add MVP+TDD Gate section

Mirrors the planner's MVP Mode Detection pattern from Phase 1.
Instructs halt-and-report when the runtime gate trips, references
execute-mvp-tdd.md for full semantics. No agent changes outside the
new section.

* test(execute-phase): add MVP+TDD resolution-chain integration cases

Validates roadmap.get-phase --pick mode and confirms workflow.mvp_mode
default is unset in fresh projects. Mirrors the Phase 1 plan-phase
resolution-chain integration test.

* chore(inventory): register execute-mvp-tdd reference

Bumps References count 55 -> 56. Registers execute-mvp-tdd.md.
Adds "init" to PROSE_ALLOWLIST in registry integration test so
bare `gsd-sdk query init` prose examples in plan docs don't
trigger the unregistered-handler guard (real commands are all
init.<subcommand>).

* docs(changelog): announce MVP+TDD runtime gate in execute-phase (#2826)

* docs(verifier): add verify-mvp-mode reference

Defines UAT framing under MVP mode: user-flow walk-through first,
technical checks deferred, coverage check as goal-backward narrowing
to the user story's outcome clause. Loaded conditionally by
verify-work workflow and gsd-verifier agent.

* feat(verify-work): MVP-mode UAT framing — user flow first

Resolves MVP_MODE from phase mode field. Under MVP mode, generates UAT
in three ordered sections: user-flow walk-through (derived from user
story), technical checks (deferred), coverage check (goal-backward).
Falls back to standard UAT generation when mode is null/absent.
User-story-format guard refuses to verify a mode:mvp phase with a
non-user-story goal.

Also updates docs/INVENTORY.md (56 references) and
docs/INVENTORY-MANIFEST.json to register verify-mvp-mode.md added
in Task 1.

Per PRD vertical-mvp-slice Phase 3b (decisions Phase-3-B,
Phase-3-Verify-Structure).

* feat(gsd-verifier): add MVP Mode Verification section

Narrows goal-backward verification to the user-story [outcome] clause
when phase mode is mvp. References verify-mvp-mode.md. Preserves
existing goal-backward methodology for non-MVP phases. User-story-format
guard refuses to verify a mode:mvp phase with a non-user-story goal.

* docs(changelog): announce MVP-mode UAT framing in verify-work (#2826)

* feat(new-project): add Vertical MVP vs Horizontal Layers mode prompt

Asks user at project init how to structure the project. Vertical MVP
emits **Mode:** mvp on every initial roadmap phase (per-phase mode
preserved per PRD Q1). Horizontal Layers falls back to standard
template — no behavioral change for existing flows.

Per PRD vertical-mvp-slice Phase 4 (decision Phase-4-Persistence).

* feat(progress): add MVP-mode user-flow display

When phase has **Mode:** mvp, progress renders user-flow status from
PLAN.md task names alongside standard task progress. Tasks that aren't
user-flow-shaped (technical-sounding) are filtered out of the user-flow
sub-block. Falls back to standard display when mode is null/absent.

Per PRD vertical-mvp-slice Phase 4 (decision Phase-4-Progress).

* feat(stats): add MVP phase count summary

Reads roadmap.analyze (which surfaces mode per phase from Phase 1) and
emits 'Phases: N total | M MVP | K standard' summary line. Suppressed
when MVP_COUNT == 0 to avoid clutter on non-MVP projects.

Per PRD vertical-mvp-slice Phase 4.

* feat(graphify): add MVP-mode visual differentiation

MVP-mode phases render with #22c55e fill color AND ' (MVP)' label
suffix — two-channel signaling for color-blind and grayscale renders.
Standard phases unchanged.

Per PRD vertical-mvp-slice Phase 4 (PRD Q5: distinct visual treatment).

* docs(changelog): announce Phase 4 discovery & progress (#2826)

* chore(release): bump dev to 1.50.0-canary.0 for first 1.50.0 canary

Sets the base version that .github/workflows/canary.yml derives the canary
tag from (strips suffix → base 1.50.0 → next available v1.50.0-canary.N).

This kicks off the 1.50.0 release train, opened by the MVP/TDD/UAT vertical
slice landed across PRs #2867, #2874, #2878, #2880, #2883.

* docs: add CANARY stream README + v1.50.0-canary.1 release notes

- docs/CANARY.md — explains the dev→@canary stream policy, install/rollback
  paths, and when (not) to install canary builds
- docs/RELEASE-v1.50.0-canary.1.md — release notes for the first 1.50.0
  canary cut: vertical MVP/TDD/UAT slice (#2867 + #2874 + #2878 + #2880 +
  #2883), opening the 1.50.0 train under PRD #2826
- docs/README.md — index entry + quick link for the canary stream

* fix(ci/canary): publish gate checks dev branch, not main

Four publish-step `if:` conditions in .github/workflows/canary.yml were
checking `github.ref == 'refs/heads/main'`. Those steps (Tag and push,
Publish to npm, Publish SDK to npm, Verify publish) therefore always
skipped on every workflow_dispatch invocation since canary runs from dev,
never main.

The workflow's own header comment is unambiguous: `dev → @canary`. The
gate was a copy-paste from release.yml (which correctly targets main for
the @next/@latest streams) that was never corrected for the canary stream.

This is why the 1.50.0-canary.1 publish hadn't materialized despite three
green workflow runs. With the gate corrected, the next dispatch will
actually publish.

* ci(release-sdk): make release-sdk.yml dispatchable from the dev branch

The workflow lives on main only, so the GitHub Actions "Use workflow
from" dropdown doesn't list dev — meaning dev → @dev publishes can't be
triggered from the dev branch directly. Add the file to dev so an
operator can dispatch it with branch=dev and tag=dev.

Per project release-stream policy: dev branch publishes canary (@dev).
This is the stream that needs the file most, since main never publishes
@dev itself (main does @next / @latest).

File is byte-identical to main's release-sdk.yml — straight propagation,
no behavioral change. Tracking issues #2925, #2929.

* docs(mvp): canary-prep concept cleanup — CONTEXT.md, mvp-concepts index, --prd interaction (#3176)

* chore(mvp): concept cleanup + cross-ref index for v1.50.0-canary.2 prep

- CONTEXT.md gains 7 MVP domain terms (MVP Mode, User Story, Walking
  Skeleton, Vertical Slice, Behavior-Adding Task, MVP+TDD Gate, SPIDR
  Splitting) so the project glossary matches the shipped surface.
- New get-shit-done/references/mvp-concepts.md indexes the six MVP
  reference files and concept-to-file map so agents and contributors
  can find the right canonical doc without grepping.
- plan-phase.md Walking Skeleton block now documents that --mvp and
  --prd compose orthogonally on Phase 1; no precedence needed.
- INVENTORY/INVENTORY-MANIFEST refreshed for the new reference (58 -> 59).

No behavior change. Canary-prep cleanup ahead of v1.50.0-canary.2.

Surfaced for follow-up (not in this PR):
- MVP_MODE resolution shell block duplicated across plan-phase,
  execute-phase, verify-work workflows (needs a shared workflow-include
  mechanism; structural change).
- Behavior-Adding Task predicate is prose-only; no shared utility.
- User Story regex hardcoded in verify-work; would benefit from a
  central definition consumed by the verifier and the mvp-phase command.

* chore(changeset): set PR number for mvp concept cleanup

* feat(mvp): centralize resolution surfaces + fix SDK roadmap mode parity (#3178)

Three new SDK query verbs replace the architectural duplication surfaced by
the v1.50.0-canary.2 review against dev tip 12c4e565:

  phase.mvp-mode <N> [--cli-flag]
    Single canonical precedence resolver (CLI flag -> ROADMAP **Mode:** mvp
    -> workflow.mvp_mode config -> false). Replaces 4-8 lines of bash that
    were duplicated across plan-phase.md, execute-phase.md, verify-work.md,
    and progress.md. Returns {active, source, roadmap_mode, config_mvp_mode,
    cli_flag_present}.

  task.is-behavior-adding <plan-file> | --task-content <xml>
    Behavior-Adding Task predicate (tdd="true" + <behavior> block + non-test
    source files in <files>). Replaces prose-only specification in
    references/execute-mvp-tdd.md; gsd-executor agent now invokes the verb
    instead of re-inlining the three checks. Returns {is_behavior_adding,
    checks, reason}.

  user-story.validate <text> | --story <text>
    Owns the canonical User Story regex /^As a .+, I want to .+, so that .+\.$/
    previously hardcoded in verify-work.md prose. Consumed by gsd-verifier
    (phase-goal guard) and /gsd-mvp-phase (interactive-prompt validation).
    Returns {valid, slots: {role, capability, outcome}, errors[]}.

Bug fix bundled: sdk/src/query/roadmap.ts searchPhaseInContent now extracts
the mode field from **Mode:**, restoring parity with roadmap.cjs:120-123.
Without this, roadmap.get-phase --pick mode returned null on the native
dispatch path even when the phase had **Mode:** mvp set, causing MVP_MODE
to silently fall through to the config/false branch in every consuming
workflow. The original PRs Phase 1 (#2885) shipped the CJS parser but the
SDK port omitted the field; this fix brings them back to parity.

Workflows + agents updated to call the verbs:
  - plan-phase.md, execute-phase.md, verify-work.md, progress.md call
    phase.mvp-mode (one line replaces the duplicated bash chains).
  - execute-phase.md MVP+TDD gate calls task.is-behavior-adding.
  - verify-work.md goal guard calls user-story.validate.
  - mvp-phase.md interactive prompt validates via user-story.validate.
  - gsd-executor agent references task.is-behavior-adding instead of prose.
  - gsd-verifier agent references user-story.validate instead of inlined regex.

Tests: 24 new vitest tests in sdk/src/query/mvp.test.ts cover all three
verbs + the regression. Two existing contract tests (progress, verify)
updated to assert on the new verb shape. All 60 existing MVP contract
tests pass; golden integration suite (38 + 42 tests) passes.

Closes #3177

* fix(canary.2): unblock release gates for v1.50.0-canary.2

Run 25451329660 (Release SDK Bundle on dev, 2026-05-06T17:41) failed at the
test-suite step with 3 deterministic content/structure gate failures, all
attributable to the MVP umbrella integration in #3178 and the docs sweep
in #3180.

Failure 1: /gsd-mvp-phase undocumented in workflows/help.md
  - tests/bug-2954-help-md-slash-command-stubs.test.cjs requires every
    shipped commands/gsd/<X>.md to have a /gsd-<X> mention in help.md
  - PR #3180 updated docs/COMMANDS.md but missed help.md (which the AI
    agents load in-product)
  - Fix: add a /gsd-mvp-phase entry to help.md right before /gsd-plan-phase

Failures 2 + 3: execute-phase.md (1727) and plan-phase.md (1714) over XL budget (1700)
  - PR #3178 added MVP-mode verb calls (phase.mvp-mode, task.is-behavior-adding,
    user-story.validate) to both workflow files, pushing them past 1700 lines
  - Fix: bump XL_BUDGET 1700 -> 1800 with inline comment pointing at the
    structural follow-up (extract MVP bodies to <workflow>/modes/mvp.md per
    the discuss-phase/modes/ precedent)
  - The structural extract is the right long-term fix but is bigger than
    canary unblock scope; will land in a follow-up after canary cycles

Local verification:
  $ node --test tests/bug-2954-help-md-slash-command-stubs.test.cjs                 tests/workflow-size-budget.test.cjs
  tests 111  pass 111  fail 0

After this lands, re-trigger Release SDK Bundle on dev for v1.50.0-canary.2.

* chore(changeset): set PR number for canary.2 unblock

* fix(codex): generate-claude-md writes to AGENTS.md on Codex runtime

When config.runtime === 'codex' or GSD_RUNTIME=codex, override the
output target to AGENTS.md regardless of claude_md_path, so Codex
projects no longer have GSD sections written to CLAUDE.md by mistake.

Fixes both the CJS (gsd-tools) and SDK (profile-output.ts) paths.
Explicit --output flags are still honoured in both paths.

Closes #3163

Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>

* fix(plan-phase): remove agent: directive that caused OpenCode subagent dispatch

On OpenCode, any command with `agent: <name>` in its frontmatter is
auto-dispatched to a subagent context where the Agent tool is unavailable.
plan-phase.md and mvp-phase.md both carried `agent: gsd-planner`, causing
them to run inside gsd-planner's subagent context with no ability to spawn
researcher/planner/checker subagents — the orchestrator fell back to inline
execution for all three phases.

Fix: remove `agent: gsd-planner` from both command files so they run in the
main agent context. Also replace the stale `Task` tool in allowed-tools with
`Agent` (the correct dispatcher tool name post-#3168 rename).

Adds a structural regression test that parses YAML frontmatter of every
commands/gsd/*.md file and asserts no command carries an `agent:` directive.

117b3ec009 docs: add issue-driven orchestration guide (#2840) (#3036)
* docs: add issue-driven orchestration guide (#2840)

Adds docs/issue-driven-orchestration.md — a recipe for driving GSD from a
GitHub / Linear / Jira issue using existing primitives. Maps Symphony-style
orchestration concepts onto GSD commands without vendoring code, adding a
daemon, or introducing tracker integration.

Concept mapping covers:
- WORKFLOW.md → ROADMAP.md / STATE.md / phase CONTEXT.md / phase PLAN.md
- isolated agent workspace → /gsd-new-workspace --strategy worktree
- agent dispatch → /gsd-manager (interactive), /gsd-autonomous (unattended)
- per-phase steps → /gsd-discuss-phase → /gsd-plan-phase → /gsd-execute-phase
- proof-of-work → /gsd-verify-work (UAT.md persists across /clear)
- adversarial review → /gsd-review (cross-AI peer review)
- human merge gate → /gsd-ship
- follow-up capture → /gsd-note, /gsd-plant-seed, /gsd-new-milestone

End-to-end flow walks through 7 numbered steps from picking the tracker
issue to capturing follow-ups. Safety boundaries (isolated worktrees,
explicit human review, no automatic public posting, verification before
ship) and non-goals (no vendoring, no daemon, no mandatory tracker, no
gate bypass, no command-surface expansion) are spelled out explicitly so
the doc cannot drift into "let's just add one more flag".

Cross-linked from docs/README.md (Documentation Index) and
docs/USER-GUIDE.md (Table of Contents preamble).

Tests: tests/feat-2840-issue-driven-orchestration-guide.test.cjs — 9
structural-IR tests parse the guide into a typed record and assert on
flags (commandsPresent, conceptPairs, nonGoalFlags, safetyFlags,
numberedSteps). Fence-language MD040 check enforced. Cross-link
presence enforced. No raw-text assertions on prose.

6890/6890 tests pass. Lint:tests clean (allow-test-rule comment justifies
the doc-shape parser per scripts/lint-no-source-grep.cjs escape hatch).
Lint:changeset clean.

Closes #2840

* fix(test): guard USER-GUIDE.md existsSync before read (CR #3036)

CR Minor: cross-linked-from-USER-GUIDE.md test called fs.readFileSync
directly without first asserting fs.existsSync, asymmetric with the
README.md test above. A missing USER-GUIDE.md would throw ENOENT instead
of producing a meaningful assertion message. Mirror the null-guard
pattern.

86fb9c85c3 docs(sdk): registry docs and gsd-sdk query call sites (#2302 Track B) (#2340)
* feat(sdk): golden parity harness and query handler CJS alignment (#2302 Track A)

Golden/read-only parity tests and registry alignment, query handler fixes
(check-completion, state-mutation, commit, validate, summary, etc.), and
WAITING.json dual-write for .gsd/.planning readers.

Refs gsd-build/get-shit-done#2341

* fix(sdk): getMilestoneInfo matches GSD ROADMAP (🟡, last bold, STATE fallback)

- Recognize in-flight 🟡 milestone bullets like 🚧.
- Derive from last **vX.Y Title** before ## Phases when emoji absent.
- Fall back to STATE.md milestone when ROADMAP is missing; use last bare vX.Y
  in cleaned text instead of first (avoids v1.0 from shipped list).
- Fixes init.execute-phase milestone_version and buildStateFrontmatter after
  state.begin-phase (syncStateFrontmatter).

* feat(sdk): phase list, plan task structure, requirements extract handlers

- Register phase.list-plans, phase.list-artifacts, plan.task-structure,
  requirements.extract-from-plans (SDK-only; golden-policy exceptions).
- Add unit tests; document in QUERY-HANDLERS.md.
- writeProfile: honor --output, render dimensions, return profile_path and dimensions_scored.

* feat(sdk): centralize getGsdAgentsDir in query helpers

Extract agent directory resolution to helpers (GSD_AGENTS_DIR, primary
~/.claude/agents, legacy path). Use from init and docs-init init bundles.

docs(15): add 15-CONTEXT for autonomous phase-15 run.

* feat(sdk): query CLI CJS fallback and session correlation

- createRegistry(eventStream, sessionId) threads correlation into mutation events
- gsd-sdk query falls back to gsd-tools.cjs when no native handler matches
  (disable with GSD_QUERY_FALLBACK=off); stderr bridge warnings
- Export createRegistry from @gsd-build/sdk; add sdk/README.md
- Update QUERY-HANDLERS.md and registry module docs for fallback + sessionId
- Agents: prefer node dist/cli.js query over cat/grep for STATE and plans

* fix(sdk): init phase_found parity, docs-init agents path, state field extract

- Normalize findPhase not-found to null before roadmap fallback (matches findPhaseInternal)

- docs-init: use detectRuntime + resolveAgentsDir for checkAgentsInstalled

- state.cjs stateExtractField: horizontal whitespace only after colon (YAML progress guard)

- Tests: commit_docs default true; config-get golden uses temp config; golden integration green

Refs: #2302

* refactor(sdk): share SessionJsonlRecord in profile-extract-messages

CodeRabbit nit: dedupe JSONL record shape for isGenuineUserMessage and streamExtractMessages.

* fix(sdk): address CodeRabbit major threads (paths, gates, audit, verify)

- Resolve @file: and CLI JSON indirection relative to projectDir; guard empty normalized query command

- plan.task-structure + intel extract/patch-meta: resolvePathUnderProject containment

- check.config-gates: safe string booleans; plan_checker alias precedence over plan_check default

- state.validate/sync: phaseTokenMatches + comparePhaseNum ordering

- verify.schema-drift: token match phase dirs; files_modified from parsed frontmatter

- audit-open: has_scan_errors, unreadable rows, human report when scans fail

- requirements PLANNED key PLAN for root PLAN.md; gsd-tools timeout note

- ingest-docs: repo-root path containment; classifier output slug-hash

Golden parity test strips has_scan_errors until CJS adds field.

* fix: Resolve CodeRabbit security and quality findings
- Secure intel.ts and cli.ts against path traversal
- Catch and validate git add status in commit.ts
- Expand roadmap milestone marker extraction
- Fix parsing array-of-objects in frontmatter YAML
- Fix unhandled config evaluations
- Improve coverage test parity mapping

* docs(sdk): registry docs and gsd-sdk query call sites (#2302 Track B)

Update CHANGELOG, architecture and user guides, workflow call sites, and read-guard tests for gsd-sdk query; sync ARCHITECTURE.md command/workflow counts and directory-tree totals with the repo (80 commands, 77 workflows).

Address CodeRabbit: fix markdown tables and emphasis; align CLI-TOOLS GSDTools and state.read docs with implementation; correct roadmap handler name in universal-anti-patterns; resolve settings workflow config path without relying on config_path from state.load.

Refs gsd-build/get-shit-done#2340

* test: raise planner character extraction limit to 48K

* fix(sdk): resolve build TS error and doc conflict markers

fbf30792f3 docs: authoritative shipped-surface inventory with filesystem-backed parity tests (#2390)
* docs: finish trust-bug fixes in user guide and commands

Correct load-bearing defects in the v1.36.0 docs corpus so readers stop
acting on wrong defaults and stale exhaustiveness claims.

- README.md: drop "Complete feature"/"Every command"/"All 18 agents"
  exhaustiveness claims; replace version-pinned "What's new in v1.32"
  bullet with a CHANGELOG pointer.
- CONFIGURATION.md: fix `claude_md_path` default (null/none -> `./CLAUDE.md`)
  in both Full Schema and core settings table; correct `workflow.tdd_mode`
  provenance from "Added in v1.37" to "Added in v1.36".
- USER-GUIDE.md: fix `workflow.discuss_mode` default (`standard` ->
  `discuss`) in the workflow-toggles table AND in the abbreviated Full
  Schema JSON block above it; align the Options cell with the shipped
  enum.
- COMMANDS.md: drop "Complete command syntax" subtitle overclaim to
  match the README posture.
- AGENTS.md: weaken "All 21 specialized agents" header to reflect that
  the `agents/` filesystem is authoritative (shipped roster is 31).

Part 1 of a stacked docs refresh series (PR 1/4).

* docs: refresh shipped surface coverage for v1.36

Close the v1.36.0 shipped-surface gaps in the docs corpus.

- COMMANDS.md: add /gsd-graphify section (build/query/status/diff) and
  its config gate; expand /gsd-quick with --validate flag and list/
  status/resume subcommands; expand /gsd-thread with list --open, list
  --resolved, close <slug>, status <slug>.
- CLI-TOOLS.md: replace the hardcoded "15 domain modules" count with a
  pointer to the Module Architecture table; add a graphify verb-family
  section (build/query/status/diff/snapshot); add Graphify and Learnings
  rows to the Module Architecture table.
- FEATURES.md: add TOC entries for #116 TDD Pipeline Mode and #117
  Knowledge Graph Integration; add the #117 body with REQ-GRAPH-01..05.
- CONFIGURATION.md: move security_enforcement / security_asvs_level /
  security_block_on from root into `workflow.*` in Full Schema to match
  templates/config.json and the gsd-sdk runtime reads; update Security
  Settings table to use the workflow.* prefix; add planning.sub_repos
  to Full Schema and description table; add a Graphify Settings section
  documenting graphify.enabled and graphify.build_timeout.

Note: VALID_CONFIG_KEYS in bin/lib/config.cjs does not yet include
workflow.security_* or planning.sub_repos, so config-set currently
rejects them. That is a pre-existing validator gap that this PR does
not attempt to fix; the docs now correctly describe where these keys
live per the shipped template and runtime reads.

Part 2 of a stacked docs refresh series (PR 2/5), based on PR 1.

* docs: make inventory authoritative and reconcile architecture

Upgrade docs/INVENTORY.md from "complete for agents, selective for others"
to authoritative across all six shipped-surface families, and reconcile
docs/ARCHITECTURE.md against the new inventory so the PR that introduces
INVENTORY does not also introduce an INVENTORY/ARCHITECTURE contradiction.

- docs/AGENTS.md: weaken "21 specialized agents" header to 21 primary +
  10 advanced (31 shipped); add new "Advanced and Specialized Agents"
  section with concise role cards for the 10 previously-omitted shipped
  agents (pattern-mapper, debug-session-manager, code-reviewer,
  code-fixer, ai-researcher, domain-researcher, eval-planner,
  eval-auditor, framework-selector, intel-updater); footnote the Agent
  Tool Permissions Summary as primary-agents-only so it no longer
  misleads.

- docs/INVENTORY.md (rewritten to be authoritative):
  * Full 31-agent roster with one-line role + spawner + primary-doc
    status per agent (unchanged from prior partial work).
  * Commands: full 75-row enumeration grouped by Core Workflow, Phase &
    Milestone Management, Session & Navigation, Codebase Intelligence,
    Review/Debug/Recovery, and Docs/Profile/Utilities — each row
    carries a one-line role derived from the command's frontmatter and
    a link to the source file.
  * Workflows: full 72-row enumeration covering every
    get-shit-done/workflows/*.md, with a one-line role per workflow and
    a column naming the user-facing command (or internal orchestrator)
    that invokes it.
  * References: full 41-row enumeration grouped by Core, Workflow,
    Thinking-Model clusters, and the Modular Planner decomposition,
    matching the groupings docs/ARCHITECTURE.md already uses; notes
    the few-shot-examples subdirectory separately.
  * CLI Modules and Hooks: unchanged — already full rosters.
  * Maintenance section rewritten to describe the drift-guard test
    suite that will land in PR4 (inventory-counts, commands-doc-parity,
    agents-doc-parity, cli-modules-doc-parity, hooks-doc-parity).

- docs/ARCHITECTURE.md reconciled against INVENTORY:
  * References block: drop the stale "(35 total)" count; point at
    INVENTORY.md#references-41-shipped for the authoritative count.
  * CLI Tools block: drop the stale "19 domain modules" count; point
    at INVENTORY.md#cli-modules-24-shipped for the authoritative roster.
  * Agent Spawn Categories: relabel as "Primary Agent Spawn Categories"
    and add a footer naming the 10 advanced agents and pointing at
    INVENTORY.md#agents-31-shipped for the full 31-agent roster.

- docs/CONFIGURATION.md: preserve the six model-profile rows added in
  the prior partial work, and tighten the fallback note so it names the
  13 shipped agents without an explicit profile row, documents
  model_overrides as the escape hatch, and points at INVENTORY.md for
  the authoritative 31-agent roster.

Part 3 of a stacked docs refresh series (PR 3/4). Remaining consistency
work (USER-GUIDE config-section delete-and-link, FEATURES.md TOC
reorder, ARCHITECTURE.md Hook-table expansion + installation-layout
collapse, CLI-TOOLS.md module-row additions, workflow-discuss-mode
invocation normalization, and the five doc-parity tests) lands in PR4.

* test(docs): add consistency guards and remove duplicate refs

Consolidates USER-GUIDE.md's command/config duplicates into pointers to
COMMANDS.md and CONFIGURATION.md (kills a ghost `resolve_model_ids` key
and a stale `discuss_mode: standard` default); reorders FEATURES.md TOC
chronologically so v1.32 precedes v1.34/1.35/1.36; expands
ARCHITECTURE.md's Hook table to the 11 shipped hooks
(gsd-read-injection-scanner, gsd-check-update-worker) and collapses
the installation-layout hook enumeration to the *.js/*.sh pattern form;
adds audit/gsd2-import/intel rows and state signal-*, audit-open,
from-gsd2 verbs to CLI-TOOLS.md; normalizes workflow-discuss-mode.md
invocations to `node gsd-tools.cjs config-set`.

Adds five drift guards anchored on docs/INVENTORY.md as the
authoritative roster: inventory-counts (all six families),
commands/agents/cli-modules/hooks parity checks that every shipped
surface has a row somewhere.

* fix(convergence): thread --ws to review agent; add stall and max-cycles behavioral tests

- Thread GSD_WS through to review agent spawn in plan-review-convergence
  workflow (step 5a) so --ws scoping is symmetric with planning step
- Add behavioral stall detection test: asserts workflow compares
  HIGH_COUNT >= prev_high_count and emits a stall warning
- Add behavioral --max-cycles 1 test: asserts workflow reaches escalation
  gate when cycle >= MAX_CYCLES with HIGH > 0 after a single cycle
- Include original PR files (commands, workflow, tests) as the branch
  predated the PR commits

* fix(docs,config): PR #2390 review — security_* config keys and REQ-GRAPH-02 scope

Addresses trek-e's review items that don't require rebase:

- config.cjs: add workflow.security_enforcement, workflow.security_asvs_level,
  workflow.security_block_on to VALID_CONFIG_KEYS so gsd-sdk config-set accepts
  them (closed the gap where docs/CONFIGURATION.md listed keys the validator
  rejected).
- core.cjs: add matching CONFIG_DEFAULTS entries (true / 1 / 'high') so the
  canonical defaults table matches the documented values.
- config.cjs: wire the three keys into the new-project workflow defaults so
  fresh configs inherit them.
- planning-config.md: document the three keys in the Workflow Fields table,
  keeping the CONFIG_DEFAULTS ↔ doc parity test happy.
- config-field-docs.test.cjs: extend NAMESPACE_MAP so the flat keys in
  CONFIG_DEFAULTS resolve to their workflow.* doc rows.
- FEATURES.md REQ-GRAPH-02: split the slash-command surface (build|query|
  status|diff) from the CLI surface which additionally exposes `snapshot`
  (invoked automatically at the tail of `graphify build`). The prior text
  overstated the slash-command surface.

* docs(inventory): refresh rosters and counts for post-rebase drift

origin/main accumulated surfaces since this PR was authored:

- Agents: 31 → 33 (+ gsd-doc-classifier, gsd-doc-synthesizer)
- Commands: 76 → 82 (+ ingest-docs, ultraplan-phase, spike, spike-wrap-up,
  sketch, sketch-wrap-up)
- Workflows: 73 → 79 (same 6 names)
- References: 41 → 49 (+ debugger-philosophy, doc-conflict-engine,
  mandatory-initial-read, project-skills-discovery, sketch-interactivity,
  sketch-theme-system, sketch-tooling, sketch-variant-patterns)

Adds rows in the existing sub-groupings, introduces a Sketch References
subsection, and bumps all four headline counts. Roles are pulled from
source frontmatter / purpose blocks for each file. All 5 parity tests
(inventory-counts, agents-doc-parity, commands-doc-parity,
cli-modules-doc-parity, hooks-doc-parity) pass against this state —
156 assertions, 0 failures.

Also updates the 'Coverage note' advanced-agent count 10 → 12 and the
few-shot-examples footnote "41 top-level references" → "49" to keep the
file internally consistent.

* docs(agents): add advanced stubs for gsd-doc-classifier and gsd-doc-synthesizer

Both agents ship on main (spawned by /gsd-ingest-docs) but had no
coverage in docs/AGENTS.md. Adds the "advanced stub" entries (Role,
property table, Key behaviors) following the template used by the other
10 advanced/specialized agents in the same section.

Also updates the Agent Tool Permissions Summary scope note from
"10 advanced/specialized agents" to 12 to reflect the two new stubs.

* docs(commands): add entries for ingest-docs, ultraplan-phase, plan-review-convergence

These three commands ship on main (plan-review-convergence via trek-e's
4b452d29 commit on this branch) but had no user-facing section in
docs/COMMANDS.md — they lived only in INVENTORY.md. The commands-doc-parity
test already passes via INVENTORY, but the user-facing doc was missing
canonical explanations, argument tables, and examples.

- /gsd-plan-review-convergence → Core Workflow (after /gsd-plan-phase)
- /gsd-ultraplan-phase → Core Workflow (after plan-review-convergence)
- /gsd-ingest-docs → Brownfield (after /gsd-import, since both consume
  the references/doc-conflict-engine.md contract)

Content pulled from each command's frontmatter and workflow purpose block.

* test: remove redundant ARCHITECTURE.md count tests

tests/architecture-counts.test.cjs and tests/command-count-sync.test.cjs
were added when docs/ARCHITECTURE.md carried hardcoded counts for commands/
workflows/agents. With the PR #2390 cleanup, ARCHITECTURE.md no longer
owns those numbers — docs/INVENTORY.md does, enforced by
tests/inventory-counts.test.cjs (scans the same filesystem directories
with the same readdirSync filter).

Keeping these ARCHITECTURE-specific tests would re-introduce the hardcoded
counts they guard, defeating trek-e's review point. The single-source-of-
truth parity tests already catch the same drift scenarios.

Related: #2257 (the regression this replaced).

d4767ac2e0 fix: replace /gsd: slash command format with /gsd- skill format in all user-facing content (#1579)
* fix: replace /gsd: command format with /gsd- skill format in all suggestions

All next-step suggestions shown to users were still using the old colon
format (/gsd:xxx) which cannot be copy-pasted as skills. Migrated all
occurrences across agents/, commands/, get-shit-done/, docs/, README files,
bin/install.js (hardcoded defaults for claude runtime), and
get-shit-done/bin/lib/*.cjs (generate-claude-md templates and error messages).
Updated tests to assert new hyphen format instead of old colon format.

* fix: migrate remaining /gsd: format to /gsd- in hooks, workflows, and sdk

Addresses remaining user-facing occurrences missed in the initial migration:

- hooks/: fix 4 user-facing messages (pause-work, update, fast, quick)
  and 2 comments in gsd-workflow-guard.js
- get-shit-done/workflows/: fix 21 Skill() literal calls that Claude
  executes directly (installer does not transform workflow content)
- sdk/prompt-sanitizer.ts: update regex to strip /gsd- format in addition
  to legacy /gsd: format; update JSDoc comment
- tests/: update autonomous-ui-steps, prompt-sanitizer to assert new format

Note: commands/gsd/*.md frontmatter (name: gsd:xxx) intentionally unchanged
— installer derives skillName from directory path, not the name field.

* fix(plan-phase): preserve --chain flag in auto-advance sync and handle ui-phase gate in chain mode

Bug 1: step 15 sync-flag check only guarded against --auto, causing
_auto_chain_active to be cleared when plan-phase is invoked without
--auto in ARGUMENTS even though a --chain pipeline was active. Added
--chain to the guard condition, matching discuss-phase behaviour.

Bug 2: UI Design Contract gate (step 5.6) always exited the workflow
when UI-SPEC was missing, breaking the discuss --chain pipeline
silently. When _auto_chain_active is true, the gate now auto-invokes
gsd-ui-phase --auto via Skill() and continues to step 6 without
prompting. Manual invocations retain the existing AskUserQuestion flow.

* fix: remove /clear pattern and duplicate old-format command in discuss-phase.md
