107eb8c1d9 feat(#3753): run docs guards on the PR that changes the docs they read (#3787)
A PR whose diff is entirely under docs/ runs zero tests, so a guard whose INPUT
is shipped prose cannot protect the PR lane of the diffs it exists to check. Its
only firing opportunity is after merge, on the shared branch -- which is how next
went red on dacae9273 while the PR that caused it (#3746) was green on every
check.

The docs-lint job in .github/workflows/docs-required.yml -- an ALREADY-REQUIRED
context -- now selects and runs the docs guards that read the specific docs files
the PR changed.

  scripts/docs-guard-registry.cjs    test file -> the docs paths it reads (63)
  scripts/select-docs-guards.cjs     pure (changedPaths, registry) -> test files
  scripts/lint-docs-guard-registration.cjs   drift guard, wired into lint:ci

scripts/ci-test-scope.cjs is NOT touched -- `git diff origin/next --` on it is
empty -- so #764's saving stands and its 21 pinning tests are untouched.

Selection: exact path; trailing-slash directory prefix (boundary-checked --
docs/adrenaline.md does NOT match docs/adr/, which a naive startsWith gets
wrong); and '*' for the 6 entries that walk docs/ generally or read a computed
path. Unknown maps to '*' -- guessing narrow is how a guard silently stops
running. Measured: a typo fix selects 6 of 63; docs/AGENTS.md selects 12;
docs/COMMANDS.md selects 18.

Four things this got wrong first, each found by an independent reviewer or by
probe, and each having been asserted safe in a comment:

1. The registry started as a RULE in ci-test-scope.cjs's RULES, on the theory
   that classify()'s !codeChanged normalization made it inert. True for
   docs-ONLY diffs; false for MIXED docs+code diffs, where codeChanged is true
   and the normalization never runs:

     node scripts/ci-test-scope.cjs --files "docs/a.md src/semver.cts"
       with the RULE:  25 targeted_tests
       origin/next:     3 targeted_tests

   Category error: RULES is the scoped lane's input; a docs-guard registry is a
   lane manifest for a consumer that never calls classify(). Extracted; pinned
   by value.

2. The second attempt was a dedicated workflow with paths: [docs/**]. Such a
   workflow never reports on a non-docs PR, so it can never be a required
   context without hanging every non-docs PR -- and a non-required check does not
   block a merge, so the guard would have been advisory and #3753 unfixed.
   docs-required.yml already has no paths: filter, already supplies the required
   docs-lint context, already computes docs_changed, and already ran one docs
   guard gated on it. Generalizing that step needs no ruleset edit at all.

3. The registry and the drift lint were built from ONE path-segment heuristic, so
   both were blind identically -- and blind at the guard that motivated the issue.
   The reader-call regex required a character BEFORE its keyword, so a callee
   named exactly read( / load( / parse( / doc( / file( / content( could never
   match; and only an INLINE path.join(ROOT,'docs','X.md') argument was caught,
   missing the two-step-via-variable form -- the MAJORITY spelling -- plus
   template literals and concatenation. Detector 1 fired on 14 of ~450 files, so
   35 genuine guards sat unregistered while the lint reported 0 violations,
   including cursor-reviewer (reads docs/COMMANDS.md, asserts
   .includes('--cursor')) and inventory-headings-countfree. The "accepted blind
   spot" this shipped with was the common case, not a fringe.

4. With detection fixed the true population is 115 files: 63 genuine guards, 52
   incidental. Running all 63 in a REQUIRED check on a one-line typo fix is the
   cost #764 exists to avoid -- install.test.cjs is 7840 lines and reads exactly
   one docs file, docs/AGENTS.md, for its frontmatter. Dropping it reproduces the
   bug; running it for a typo elsewhere is waste. Hence the map.

Then a second review round found six more, all fixed here:

- fragment-single-edit-propagation.install.test.cjs was EXEMPTED as
  "overlay fixture only". False: it reads the real docs/registries/eos.json and
  asserts on a registry entry name, and reads the real ADR-0001 and asserts its
  H1. A docs-only PR touching either would have gone green and red next -- #3753
  shipping again, from inside the fix for it. Now registered against both paths,
  and all 52 remaining exemptions were re-audited one by one.
- The SUITES-collision guard compared RAW registry keys, but run-tests.cjs strips
  a leading `tests/` BEFORE its suite check. So it caught 'all' and missed
  'tests/all' -- the only spelling that can actually occur, since every key
  carries the prefix. One typo would have run all 824 test files inside the
  required job. Now normalized the same way run-tests.cjs normalizes.
- The lint failed OPEN on an unreadable tests dir or candidate file: 0 violations,
  ok:true. A guard that cannot read its input must never report success.
- The exemption ratchet gated identity only, so a baselined file that later
  STARTED asserting on shipped docs stayed exempt silently -- 52 permanently blind
  files. The baseline now fingerprints the docs paths each exempted file
  references and fails when that set changes, naming what changed.
- The exemption marker was still honored inside a multi-line template literal in
  the header window. The scanner now tracks template-literal and block-comment
  state.
- `git diff --name-only | grep '^docs/'` silently dropped C-quoted non-ASCII docs
  paths, making docs_changed=false a green zero-guard check. Both call sites now
  pass -c core.quotepath=false.
- The run step was gated on hashFiles(), which a force-committed
  .docs-guard-tests.txt would satisfy. The step now rm -f's both scratch files
  first and gates on an output it sets itself.

Three empty states, deliberately distinct, because conflating them rebuilds
#3753: an empty or malformed registry HARD-FAILS; docs changed with no guard
covering them logs and skips; no docs change is already gated. The middle state
must never be expressed as an empty --files-from, which prints `no tests in suite
"all"` and exits 0 -- a green check that guarded nothing. With the current
registry that state is unreachable, because the six '*' entries always match;
the branch is kept as defensive handling for a future registry and says so.

timeout-minutes: 15 bounds the required job against a hanging fork-supplied test;
it had none. npm ci was added because the job never installed dependencies -- the
previous single-file step got away without it, the registry does not.

docs/contributing/docs-guard-registration.md documents the rule, following its
sibling cross-platform-portability-rules.md, and CONTRIBUTING.md's CI Test
Quality Checks table links to it. It is also load-bearing: without a docs/ file
in the diff this PR would not have triggered its own lane, shipping an
unexercised change to a required check.

One unrelated fix, included because this PR surfaced it and CLAUDE.md forbids
deferring a defect found while working. On this branch's first CI run,
`full test (windows-latest, 24, shard 3/3)` was CANCELLED at exactly 30 minutes;
tests were still passing 0.8s before the cancel, so it is a wall-clock timeout,
not a hang, and a cancelled job reddens `Required tests`.

The cause is not this PR's test file, which costs ~60ms. Shard composition is
unstable: adding ONE file to the unit suite reshuffled 115 of 268 files between
shards, and shard 3 drew a heavier mix. Underneath that is a real pre-existing
defect. tests/ci-test-job-timeout-budget.test.cjs requires every lane's budget to
be >= 1.5x its MEASURED cost -- "a lane that got slower must be re-budgeted, not
excused" -- and its test-full entry recorded 19m from a windows-22 shard. That is
stale. Measured on `next` with none of this PR's changes present: 26m18s (run
32614439702, windows-latest/24 shard 3/3), 23m36s and 23m17s on shard 2/3. So the
lane costs ~26m and the 30-minute cap carried 1.14x headroom, not 1.5x. The gate
had been out of compliance with its own rule; this PR was merely the file
addition that reshuffled shard 3 past the cliff.

Fixed as that file prescribes: measuredMinutes 19 -> 27 with fresh evidence, and
test-full timeout-minutes 30 -> 45. The rule's minimum for 27m is 41; 45 is
deliberately above it because the reshuffle means per-shard worst case moves run
to run, and a budget pinned to the exact minimum would be re-breached by the next
test file anyone adds. Only that one job's timeout changed; test.yml's scope,
matrix and steps are untouched, so #764's saving is unaffected.

Raising that cap let the Windows shard finish (28m45s, inside 45) and uncovered
a real failure the 30-minute cancel had been masking:
`new quick-task branch branches off origin/main (#2916)` died with
`outcome=timed_out exitCode=null`, SIGTERM, at the 15000ms bound.

tests/quick-branching.test.cjs:149 `runStep` runs a `#!/usr/bin/env bash` script
executing MULTIPLE git commands, but was bound to GIT_TIMEOUT_MS (15000) -- the
norm for a SINGLE git plumbing call. tests/helpers/timeouts.cjs already documents
this exact failure and exists to fix it: HOOK_FANOUT_TIMEOUT_MS was created after
PR #3285 recorded "outcome=timed_out exitCode=null at exactly the 15000ms probe
bound while every other lane passed the same commit", and calls that "a bound
sized for the wrong class, not a slow machine". Our failure is that case
verbatim, so both sites move to the class norm rather than to a bigger number.

The same class also failed on `next` itself 21 hours earlier -- run 32608945654,
windows-latest/24 shard 1/3, `plan touching only src/ in a submodule project
keeps worktree isolation ENABLED` -- where tests/worktree-safety.test.cjs:5845
`runGate` fans out to `git config --file .gitmodules` under a hardcoded 30000.
Fixed too, since it is a defect in the tree regardless of which branch surfaced
it.

A survey of the whole tests/ tree found the same class-mismatch at further
bash fan-out sites bound under 60000ms, and the maintainer approved sweeping
them rather than leaving them latent to surface the same way one at a time. 16
fan-out sites across 16 files now use the class norm.

The sweep is class-correctness, not raising numbers until things pass. Sites
were moved ONLY where the bash body demonstrably spawns something (git, node,
npm, a CLI); self-contained shell snippets were left where they are, and are
listed as deliberately unchanged: pure if/printf bodies (copilot-install), pure
array/case builtins (code-review-pipeline-regression:638), a documented
pure-shell gsd_run stub (host-integration), single-process hook calls
(workflow-guard:222/271/302), and a deliberately tight 5000ms fast-check hook
(gsd-write-guard.property). Nothing was lowered. process-seam.test.cjs:513
(literal 300) is untouched on purpose -- it tests timeout BEHAVIOR, so raising
it would destroy what it asserts.

Shared file-level constants were the trap here, and were handled per file rather
than by redefinition: GIT_TIMEOUT_MS has ~15 users in git-base-branch and only 1
is a fan-out; WORKTREE_TIMEOUT_MS has 16 users in worktree.test.cjs and 3 are;
PROBE_TIMEOUT_MS has several in three more files. In each the CALL SITE was
changed and the constant left alone, so no single-plumbing-call site silently
inherited a 60s bound. The one exception is hooks-opt-in.test.cjs, where
HOOK_TIMEOUT_MS has exactly one consumer -- spawnHook, the fan-out itself -- so
redefining it is identical in effect and reads better.

Only two of these sites have actually been observed failing. The rest cite that
shared class and those two run ids rather than inventing evidence of their own.

fe64704ace enhance(#3588): add an opt-in commit_docs pre-commit hook (#3609)
* feat(#3588): add an opt-in commit_docs pre-commit hook

Final phase of epic #2292, scope narrowed to opt-in by maintainer decision:
default-on installation and the bin/install.js wiring it would have required
are explicitly out of scope.

Enabling is an explicit verb call. The hook is written to the repo's real hooks
dir resolved via git rev-parse --git-path hooks, so a linked worktree or
submodule whose .git is a FILE works rather than getting a literal .git/hooks
path. It refuses rather than overwrite a foreign pre-commit, refuses to delete
one it did not write, and refuses outright when core.hooksPath is already set --
a written-but-ignored hook is worse than a refusal. Ownership is detected by
marker presence, not byte-equality, so a user who appends a line does not make
it unrecognizable.

Deliberately NOT included: teaching cmdCheckCommit the per-phase commit_docs
tier. #3587 was still unmerged when this landed, and implementing precedence
against helpers that did not yet exist would have meant a second copy of the
resolution chain -- the divergence class this epic has spent three phases
fighting. That follows as its own change now that #3587 is on next.

The ordering constraint is recorded in the design doc: this must not merge
before #3587, or the hook would block a commit cmdCommit itself allows.

* fix(#3588): teach the commit_docs guard the per-phase tier and -z paths

Part 1, deferred until #3587 merged.

cmdCheckCommit read only project-level commit_docs, so once #3587 landed, a
phase with phase_commit_docs true under project false was ALLOWED by
query commit and BLOCKED by this guard -- and the hook shipped in this same
branch shells out to it. It now derives the staged phase via the single-owner
detectPhaseNumberFromFiles and resolves through #3587's own
resolveCommitDocsPolicy rather than a second precedence copy.

Also fixes a proven false negative in the harm direction. git diff --cached
--name-only C-style-quotes any path with non-ASCII or special characters, so a
staged .planning/cafe.md was emitted as a quoted string, failed
startsWith('.planning/'), and slipped past the guard entirely under
commit_docs:false. Reading with -z and splitting on NUL removes the quoting at
the source. The f.startsWith('.planning\\') branch was dead code under that
read -- git emits /-separated paths on every platform -- and is removed rather
than left implying coverage it never provided.

The earlier C7 test pinned the buggy behavior as intended; it now asserts the
file is detected and the commit refused.

Self-caught: the commit-docs-guard verb was wired into the routers by this
branch's earlier pass but missing from the top-level help listing.

* test(#3588): replace try/finally with t.after, add negative-routing cases

Standards review findings.

CONTRIBUTING bans try/finally inside a test body outright -- it masks failures
-- and B8 used one for worktree cleanup. Now t.after(), assertions unchanged.

The new commit-docs-guard command family had zero negative-routing coverage,
which CONTRIBUTING requires for any change to command dispatch. B11-B15 cover
no subcommand, unknown, empty string, whitespace-only and a flag-shaped value,
each asserting non-zero exit, a structured error, no stack trace, and -- the
one that matters for a command that writes into a user's repo -- that NO hook
is written in any of them.

Those tests were verified to fail when routeCommitDocsGuard's else-branch is
neutered, so they exercise the routing guard rather than any convenient error
path.

Also made two error() calls' control flow explicit with a return; they were
safe only because error() is typed never two files away.

* chore(#3588): backfill changeset pr number to 3609

* test(#3588): skip Windows-unrepresentable fixtures on win32

CI's Windows shards caught two of my own tests: fixtures whose filenames
contain a quote and a backslash. Both are illegal on Windows -- backslash is
the path separator, quote is invalid on NTFS -- so fixture creation failed
before any assertion ran.

Test-portability defect, not a production one. Those inputs cannot exist on
that platform, so the guard has nothing to detect there.

Both now check process.platform FIRST, before any fs or git call, and use
t.skip() rather than a bare return -- a bare return registers as a PASS and
would hide the gap it is meant to record. Each carries a comment saying the
input is unrepresentable rather than unverified, so nobody later re-enables it.

No padding added: the cafe.md case already exercises git's C-quoting path on
every platform, since non-ASCII names are legal on NTFS.

This is exactly the coverage the Linux-only remote matrix cannot provide, which
the PR body already stated -- CI's Windows shards are what caught it.

ddf852873c fix(#3357): one phase-pinned resolver for verification-report discovery (#3513)
A phase directory can hold more than one `*-VERIFICATION.md` — an ad-hoc `03-CORRECTION-VERIFICATION.md` worksheet beside the real `03-VERIFICATION.md`. Discovery took the alphabetically-first match, so the worksheet won and the phase could report `missing` while a passing report sat next to it.

The issue named two copies. There were seven, in four grammars: two `.sort()[0]` sites in the verification module, three `.find()` over UNSORTED readdir order (phase status, and `verification_path` twice — filesystem-dependent, so two machines on one commit could disagree), and two in shell. All seven now route through one exported `resolveVerificationFile`; the shell copies via a new `verification resolve-file` verb rather than hand-rolling the rule an eighth time.

Fixing five of seven would have been worse than fixing none: the verify-work workflow is a WRITER that stamps `status: passed` onto the file it picks, so canonical-aware readers plus an alphabetical writer means the human_needed→passed canonicalization silently no-ops forever while the worksheet gets stamped. That divergence did not exist on next.

The first resolver was itself a regression — it preferred ANY canonically-shaped name over the phase's own report, so a stray cross-phase or sentinel-numbered file outranked it. The global-canonical preference was removed rather than narrowed; the rule is pinned to the phase token via `PHASE_NUMBER_TOKEN_SOURCE`, its existing owner.

Fixed in passing: the transition workflow's awk guarded on `NR==1` instead of `FNR==1`, so across a multi-file glob it armed only on the first file — a leading worksheet with no frontmatter blocked transition even when the canonical report passed. Also removed a U+00AD soft hyphen introduced earlier on this branch.

Five broader-grammar AGGREGATE scans are deliberately out of scope — a different defect class (phase-unscoped scanning), tracked as #3511.

69e7afd0c7 chore(#3212): bounded quantifiers over document content — prohibition with teeth — Phase 4 (#3441)
* feat(#3415): ship local/no-unbounded-quantifier, burn down ReDoS class

Phase 4 of epic #3212 (ADR-3212 §5/§7, the final phase). New rule flags
an unbounded */+/{n,} quantifier over a broad character class
([\s\S], dotAll ., or a 1-2-unit negated class like [^\n]/[^)\n] — the
exact #2128-fixed shape) applied to a regex whose match target is
data-flow-traced to readFileSync content.

eslint-rules/lib/readfilesync-trace.cjs extracts the data-flow tracer
shared with no-crlf-fragile-split (Phase 2) rather than a second copy
— no-crlf-fragile-split refactored onto it with zero behavior change,
parity-tested.

Real triage, not 798 mechanical edits: the ADR's census (2026-08-08)
screened every unbounded quantifier in the tree unscoped. Correctly
scoped to readFileSync-derived content (matching Phase 2's own G2/G3
scoping), the rule found 162 real hits across two detection waves — the
second wave (93) surfaced only after a genuine off-by-one bug in this
rule's own first draft was caught while writing its RuleTester tests
and fixed (the bug silently missed every directly-quantified [\s\S]*
with no gap before the quantifier — exactly the class this rule exists
to catch). 3 hits landed in production src/ (commands.cts, milestone.cts,
roadmap.cts) and were each empirically timed against adversarial input
(matching #2128's own measured-not-assumed precedent) — all confirmed
linear-time/benign, left unbounded with a measured-evidence comment
rather than mechanically bounded. The remaining 159 are test-file
fixture parsing (test-author-controlled, fixed-size content, not
adversarial input) — each suppressed with a specific, non-generic
reason. Zero functional behavior changed anywhere in this diff.

tests/no-pending-3212-markers.test.cjs locks the epic's own closing
invariant (ADR §7: "assert zero pending #3212 markers remain") — ground
truth confirmed trivially true today (no phase left any such marker
behind), now regression-locked going forward.

An orthogonal Standards-axis review found eslint-rules/no-unbounded-quantifier.cjs
mistakenly carried meta.docs.category: 'Portability', copied from a sibling
rule without realizing what that implied: docs/contributing/cross-platform-
portability-rules.md governs an ADR-1703 rule family under a hard "zero
escape hatches" contract (tests/portability-rule-disable-ban.test.cjs's
PROTECTED_RULES bans eslint-disable for those rules entirely). This rule is
not part of that family — it's ADR-3212 (ReDoS/CWE-1333), a different epic —
and its eslint-disable-next-line suppressions (159 of them, added earlier
this same phase after empirical benign-verification) are an intentional,
correct design, not a bypass. Corrected to category: 'Best Practices',
matching the actual precedent (no-adhoc-regex-escape.cjs, Phase 1 of the
same epic, which is also correctly outside PROTECTED_RULES), and the rule's
own docstring now states this explicitly so a future reader doesn't have to
re-derive it.

Also registers a new scripts/ci-test-scope.cjs bucket so editing this rule
or the shared eslint-rules/lib/readfilesync-trace.cjs helper re-runs their
own test suites under targeted CI selection — was previously unregistered
and invisible to that fast-path (this PR's own gsd-test checkpoint runs the
full suite regardless, so this only affects future narrowly-scoped PRs).

* fix(#3415): bound no-unbounded-quantifier's own scanner (CWE-1333, ironic)

Security review found the rule meant to catch algorithmic-complexity bugs
had one of its own: hasUnboundedBroadQuantifier's negated-class inner
scan walked from each `[^` occurrence to the next `]` (or EOF) with no
bound, while the outer loop only ever advanced by one character — O(n²)
total work on a pattern with many unclosed `[^` runs. Runs unconditionally
inside checkPattern on any `new RegExp('literal string')` argument in any
linted file, before the (cheap) readFileSync data-flow gate — so a single
crafted string literal, no valid regex syntax required, could make
`npm run lint` / CI hang.

Empirically confirmed both the bug and the fix: pre-fix, n=4000/8000/
16000/32000 chars took 30.8/115.6/463.8/1874.3ms (~4x work per 2x n,
quadratic); extrapolated, the 300000-char repro from the finding would
run ~165s. Post-fix (bail the inner scan once units exceeds the rule's
own 1-2-unit scope, rather than continuing to hunt for a closing `]`),
the same 300000-char input runs in 8.7ms via the real rule module,
independently reconfirmed at 18ms via a fresh Linter.verify() call.

New regression row in tests/no-unbounded-quantifier.rule.test.cjs
asserts the RuleTester run on a 50000-char adversarial pattern
completes and returns a defined result — no wall-clock assertion
(CLAUDE.md Clock Seams / local/no-elapsed-assertion).

  already triaged elsewhere in this PR. Suppressed with the same
  established reason.
- lint-allow-test-rule-refs' ratchet ceiling needed re-raising again
  (301 -> 303) for the same reason as the two prior bumps: organic
  growth from unrelated, already-reviewed PRs landing concurrently,
  not a defect in this branch's own diff.

e7993d77bf fix(#3207): create-and-switch on the first phase/milestone commit (#3363)
* test(#3207): invert fresh-create branching tests to expect create+switch

The four fresh-create branching tests (#3079) locked in create-without-
switch behavior that #3207 identifies as the regression: a fresh phase/
milestone branch is created but HEAD never moves onto it, so the first
phase/milestone-scoped commit lands on the base branch. Invert those
assertions to expect create+switch, and add two new tests:
- fresh-create is non-silent (logs the create+switch) (#3207 AC3)

The existing-branch path (#2539) is deliberately byte-for-byte unchanged.

* fix(#3207): create-and-switch on the first phase/milestone commit

The #3079 fix (PR #3141) replaced git checkout -b with git branch
(create-only) unconditionally, including the case where the strategy
branch does not yet exist. That regressed #1278: the first phase- or
milestone-scoped commit no longer landed on the strategy branch — it
stayed on the base branch, and the strategy branch was left as an empty
marker pointing at the pre-phase tip. Every subsequent commit then
warned 'already exists; committing on the current branch instead of
switching', wording that misleads because the tool itself created the
branch moments before.

Re-separate the two cases #3079 collapsed:
- branch does NOT exist -> create AND switch (git checkout -b). The
  #3079 resurrection hazard cannot apply: the branch was just verified
  absent, so there is no merged-and-deleted ref to resurrect and no
  silent move onto an existing unrelated branch (#2539 AC2 is honored
  by the existing-branch arm, which is unchanged). The create is logged
  so the first phase-scoped commit is not silent (#3207 AC3).
- branch ALREADY exists -> unchanged: no switch, commit on current
  branch, non-silent warning (#2539/#3079).

Once the first commit switches HEAD onto the strategy branch, the
currentBranch !== branchName guard skips the block on subsequent
commits, so the misleading 'already exists' warning no longer recurs.

cmdInitQuick's quick_id generation (init.cts), and reapStaleTempFiles
(io.cts). The CLI-subprocess-pinned tests are expected RED until a
follow-up commit routes those call sites through realClock so
GSD_TEST_MODE+GSD_NOW_MS can reach them.

* refactor(#3314): route CLI-subprocess clock reads through realClock

cmdCurrentTimestamp, cmdInitManager's is_active gate, and cmdInitQuick's
quick_id generation read Date directly, which the GSD_TEST_MODE+GSD_NOW_MS
subprocess pin cannot reach (it only fires inside realClock.now()).
Behavior-preserving: realClock.now() falls through to Date.now() whenever
GSD_TEST_MODE is unset, which is every real invocation.

* docs(#3314): amend ADR-456 with reachability-based clock-control rule

ADR-456 §(a) documented one mechanism (injected {clock=Date} +
t.mock.timers). Adds the two this repo already relies on: t.mock.timers
for in-process direct-Date reads, and the GSD_TEST_MODE+GSD_NOW_MS
subprocess pin (routed through realClock) for CLI-spawned code. Updates
TESTING-STANDARDS.md's matching passages, which already referenced this
issue by number as the no-elapsed-assertion promotion precondition.

* fix(#3314): address orthogonal review findings

Spec-axis findings: file and link the no-elapsed-assertion promotion
follow-up (#3331) instead of leaving TESTING-STANDARDS.md pointing at
a dead #1885, and ship the module-by-module audit table in the ADR
itself rather than only in a gitignored phase artifact.

Standards-axis finding: pin the "hour-old file = not active" test via
GSD_TEST_MODE+GSD_NOW_MS for consistency with the reachability rule
this PR's own ADR amendment now documents.

aceea3ce4a refactor(#3217): withhold a percentage when its scope is not complete (#3318)
* wip(#3217): rule-4 scope withholding — parked, two open findings

Implemented but NOT shippable. An isolated review found buildStateFrontmatter
still hardcodes SCOPE.COMPLETE, so state json reports percent 0 where roadmap
analyze, stats and query progress all correctly report null on the same disk
state - rule 4 reintroduced at a site this phase claims to close. Also: roadmap
analyze emits scope complete beside progress_percent null with nothing
explaining it.

Parked to build Phase 4 (#3186) first, which is unblocked. Findings recorded in
.gsd/phase/refactor-3217-completion-ratio-scoping/60-review.json.

* fix(#3217): withhold the sync percentage on a non-complete scope

The parked blocker is fixed - buildStateFrontmatter no longer hardcodes
SCOPE.COMPLETE, and the prose Progress fallback is gated too, which was a second
leak found while tracing the first. roadmap analyze exposes progress_scope so a
consumer can tell WHY a percentage is absent from the JSON alone.

Then a residual gap was reproduced rather than assumed. cmdStateSync carried the
same hardcode behind a written reason claiming it did not reproduce. It did: on a
TRUNCATED window and on UNSCOPED row 4, state sync wrote Progress 0 percent to 100
percent while state json, roadmap analyze, stats and query progress all withheld -
and it persisted a self-contradictory file, body claiming 100 percent while its own
frontmatter correctly omitted percent.

The excuse was also wrong. syncRoadmapRaw is already parsed in that function and is
exactly what produces a real scope, so there was a scope to pass. Threaded through
listMilestonePhaseDirs; a non-complete scope now skips the write with a reason in
changes. milestoneBounded stays as the orthogonal 1761 guard for row 5.

Second time this epic a does-not-reproduce claim was too generous. Recorded in
ADR Amendment 8 as a correction rather than a quiet rewrite.

Verified on the remote runner.

* test(#3217): give the withholding fixtures a resolvable scope

40 matrix failures, all fixture drift - no code regression. My own hypothesis
that this was over-withholding was wrong and is recorded as such: the worry case,
a plain ROADMAP with Phase entries and no version heading, resolves to complete
exactly as ADR 7.1 says it should.

The real causes were two fixture shapes. Most had no ROADMAP.md at all, which is
unreadable via a pre-existing graceful path, and asserted a numeric percent. The
five vscode, pi-extension, mcp-server and shell-projection failures were that
shape - bare temp dirs using progress json as a reachability proxy while
asserting typeof percent is number, which under rule 4 is now null.

The rest had a version token in a title or heading with no STATE.md milestone
pointer to resolve it, which is classification row 4, versioned but unresolved,
so withholding is correct per the contract.

Verified on the remote runner.

* test(#3217): make the LM-tools reachability tests dispatch against their fixture

The gsd_progress reachability test was never testing its fixture. invoke()
resolves cwd from vscode.workspace.workspaceFolders by design (the real
LanguageModelToolInvocationOptions has no cwd field, per the 2103 fix in
extension.js), the mock had no workspace at all, and the test passed a cwd option
nothing reads - so it dispatched against the repo working directory. Writing a
ROADMAP into the temp dir had no effect. Rule 4 only made it visible.

Fixed by mocking workspaceFolders. The two siblings in the same file carried the
identical dead cwd and were dispatching against the repo too; they were not
failing only because their assertions did not touch scope-dependent output. Both
now use their own fixture with assertions unchanged - the no-planning fallback
paths already satisfy them honestly.

Re-scanned the other five reachability files: no further instances. They thread
cwd into parameters that genuinely read it, not through an options shape that
ignores it.

Verified on the remote runner.

pr:0 placeholder replaced with the real number now that #3318 exists.

* ci(#3217): give the coverage merge enough heap for the merged shards

The coverage gate OOMed at exit 134. c8 report merges three shard artifacts,
roughly 358MB of V8 dumps in coverage/tmp, and died holding their per-file
position maps at the ~4GB default heap. Verified as this branch's delta rather
than pre-existing: the same job succeeded on next at 14:18, after phases 4 and 5
merged.

Both coverage-gate steps get the bump because both re-slice the same merged data.
8192 doubles what failed and leaves headroom on a 16GB ubuntu runner, matching
the idiom the shard step already uses at 6144.

This is a memory bound, not a change to what is measured. No threshold was
touched. The test file was checked for gratuitous subprocess spawning and is
already reasonable at 43 spawns, each a distinct fixture-by-surface pairing.

Verified on the remote runner.

Implements ADR-2313 D7, and owns the Codex .toml typed IR that Phase 1's
review assigned to this phase.

The IR exists for a structural reason, not tidiness: this phase has to
PARSE these files, and a parser kept bug-compatible with a separate
renderer is the generative-fix-divergence shape this epic already dealt
with once for the model predicate. So Phase 2's parsing MOVES here
rather than being copied — agent-install-check now imports it, and its
test file passing unchanged is the proof the extraction altered nothing.

The load-bearing property is byte-identical round-trip: render(parse(x))
=== x. Without it a sync silently reformats a user's file — line
endings, key order, BOM, trailing newline — turning a two-line repair
into a whole-file diff in their dotfile repo. The IR keeps original
lines and removes targeted ones rather than reconstructing from parsed
fields, which is what makes that property hold.

It also reconciles a real contradiction between Phase 2 and ADR-2313. An
unterminated developer_instructions block: the reader excludes the rest
of the file, deliberately failing toward a false positive, because
misreading prose as a pin only wastes a user's time. The writer must
refuse, because proceeding on a malformed document rewrites it. A false
positive is the safe direction for a reader and the dangerous one for a
writer. So the parse reports the fact and the two consumers branch on
it — one parse, one truth, two policies, instead of two parsers that
agree today.

The sync leaves a legal real-Codex pin and its coupled effort untouched,
reported skipped rather than synced; strips a stale Anthropic or tier
model and an orphaned effort; keeps dry-run as the default; refuses any
file whose parse fails; and skips symlinks exactly as the Claude path
already did. The Claude path itself is byte-identical.

PARSE_REASON.NO_HEADER from the ADR's illustrative snippet is
deliberately not implemented — a missing header is legal, not an error,
so it would be a dead enum member that the enum-lock test then pins.

Two findings from an isolated review, both in the write path.

BLOCKER: mixed line endings broke the byte-identical round-trip. `eol`
was a single whole-file flag and split(/\r?\n/) discarded each line's
own terminator, so render re-joined with ONE style and normalized every
line — even with zero strips performed. A file with one CRLF line and
the rest LF came back fully converted. That falsified the A14 guarantee,
violated the design's "must not silently rewrite every line", and made
the CONTEXT.md glossary claim wrong. It was untested because A12 and B15
only cover PURE CRLF; no mixed-ending fixture existed anywhere.

Fixed by keeping each line's terminator alongside its content, so render
is a plain concatenation and a strip removes only the target line and
its own terminator. `eol` survives as informational metadata that render
never reads. Seven fixtures added for the paths nothing exercised:
mixed endings unmodified and with a strip, a lone \r, a file ending on
the block's closing ''' with no newline, multiple trailing newlines, a
BOM-only file, and an empty file.

MINOR, but it contradicted this phase's own contract: the write was
in-place open-truncate, so a failure between truncate and completion
leaves a truncated .toml — exactly what ADR-2313 says must never happen.
The Codex path now writes a sibling temp file and renames over the
target, which is atomic on one filesystem, with cleanup on failure. It
uses the repo's existing retryRenameSync rather than a hand-rolled
rename, and deliberately NOT platformWriteSync, whose normalizeContent
would mangle the very CRLF and trailing-newline bytes the round-trip
property exists to preserve.

The Claude path keeps its in-place write untouched. It has the same
shape, but changing it is not this phase's business and its tests must
stay byte-identical.

B20 previously mocked writeFileSync to throw BEFORE touching anything,
so it proved nothing about a mid-write failure — its passing comment was
true only because of how the mock was built. It now performs a real
truncated write wherever writeFileSync is called, catching both the
naive direct-to-target path and the new temp path, and asserts the
target is byte-identical afterwards with no stray temp file left.

Caught by B17, one of this phase's own tests — the suite working, not a
test problem.

Content is reconstructed as the concatenation of lines[k] +
terminators[k], so a file with no trailing newline has '' as its last
terminator. removeLine spliced out both arrays at the same index, which
is right for a middle line but wrong for the last one: it dropped the
empty terminator and left the PREVIOUS line's newline in place. A file
ending `...\nmodel = "sonnet"` with no trailing newline came back
as `...\n`, gaining a newline the user never wrote.

The new last line now inherits the removed line's terminator, so a
removal leaves the file exactly as if that line had never been written.
Removing the only line yields an empty file rather than a stray
terminator.

Both stripModel and stripReasoningEffort funnel through the one
removeLine, confirmed rather than assumed, so a single fix covers both —
including the row-B7 shape where a stale model and its orphaned effort
are removed in sequence and the second removal targets the last line.

Two of the four new cases are honestly not red-first and say so in
their comments: removing a last line that HAS a trailing newline only
exposes the bug under mixed EOL, since uniform files coincidentally
have equal terminators on both sides; and removing the only line
already degenerated correctly through Array.slice. They are kept as
guards for the new branch rather than dressed up as catches.

How-to: the Codex-400 entry added in Phase 2 told users to re-run the
installer, because that was the only repair available then. It now leads
with `effort sync` and keeps the reinstall as the alternative, with the
reason to prefer one — a reinstall regenerates the agent files
wholesale, so anyone who hand-edited theirs loses those edits. Detect,
preview, apply is now one continuous path in one place.

Reference: docs/COMMANDS.md had no `effort sync` entry at all — the same
gap `validate agents` had in Phase 2, found the same way. The entry
documents BOTH runtimes, because the command genuinely forks on runtime
and describing only the new half would misdescribe it.

The write flag is `--apply`. The design doc and test matrix both said
`--no-dry-run` throughout, which does not exist — verified against the
actual arg parser in gsd-tools.cjs before writing. Documenting a flag
that does not exist is worse than documenting nothing, because it fails
at the moment someone needs it.

Both surfaces state that only the targeted lines are removed and every
other byte is preserved. That is a user-visible guarantee rather than an
implementation note: it is the difference between a two-line diff and a
reformatted file in someone's dotfile repo, it is what the IR's
round-trip property exists to deliver, and writing it down makes it a
contract a future change has to break knowingly.

"The new last line inherits the removed line's terminator" copies the
removed line's STYLE as well as its presence. A26 uses mixed endings on
purpose — line one terminated \r\n, the model line terminated \n — so
inheriting silently rewrote line one's ending to \n. That is precisely
the defect class the mixed-EOL blocker fix existed to eliminate,
reintroduced one layer down by the fix for it.

The correct rule inherits the EMPTINESS only. If the removed line had no
terminator, the new last line loses its own, preserving "this file has
no trailing newline". Otherwise the new last line keeps its own
terminator: it is already a newline, and already the right style for
that line.

A26's assertion moved too, and that deserves saying plainly rather than
burying: it previously encoded my wrong rule. Changing a test to match
the implementation is usually the mistake, so it was checked from first
principles instead — a file whose first line ends \r\n and whose last
line ends \n, with that last line removed entirely, must be the first
line with its own \r\n intact. The new expectation is what the user's
file should actually look like; the old one was wrong.

A29 adds the interaction nothing covered: the compounding case (strip a
stale model, then its orphaned effort, the second removal landing on the
last line) with non-uniform endings either side. The two fixes meet
there and nothing exercised the meeting point.

fixes there each surfaced the next issue, which was the signal that the
data model was wrong.

splitPreservingTerminators left a phantom empty final entry for any file
ending in a newline: "a\nb\n" became lines ['a','b','']. So for the
common case the real last content line was NOT the last array element,
removeLine's isLastLine check never matched it, and every rule I gave
was reasoning about the wrong element.

What hid it: render was already a plain concatenation, so a phantom
empty line with an empty terminator contributes nothing to the output.
A14's byte-identical round-trip could never have caught it — the defect
is byte-neutral until a removal shifts the index arithmetic under it.
That is worth recording, because "the round-trip test is green" was
exactly the reassurance that kept the search pointed elsewhere.

The representation is now 1:1 — terminators[i] follows lines[i] and may
be '' — with no phantom, verified across empty, no-trailing-newline,
trailing-newline, blank-line and mixed-CRLF inputs. render stays a plain
concat and needs no special cases. With the phantom gone the removal
rule is correct as stated and finally applies to the genuinely last
element.

Consumers checked rather than assumed: the block-range detector and
header scanner are agnostic to array shape, and Phase 2's reader uses
its own independent split, so tests/agent-install-check.test.cjs is
untouched and still passes unchanged.

One test expectation was wrong and is corrected rather than quietly
adjusted: A18 asserted a 7-element terminators array whose trailing ''
was the phantom itself. It now asserts the six real terminators, which
is what the invariant lines.length === terminators.length requires.

* test(#3148): bound the long tail and delete the allowlist

Migrates the final 170 unbounded sync spawn sites across 49 files, then
removes the allowlist entirely. local/no-unbounded-spawn now runs with no
exemption surface across tests/**: there is no file to add a name to.

drift-detection's throw-native git() helper routes to gitOrThrow -- bare
runGit would have taken 16 call sites quiet on failure. commands.test.cjs
has two independently-scoped runGsdTools/runCli helpers, one already bounded
and one not; they are kept distinct rather than unified, the same trap as the
two same-named git() helpers in Wave 1.

runNpm's bound was erasable. Its options spread callerOptions after the
defaults, so an explicit timeout:undefined silently dropped the 180000ms
bound -- the rule flagged it and was right; it was not a false positive. Fixed
by destructuring with a default, with a test that fails when the default is
removed.

Two sites stay on a raw spawn with an explicit timeout because the seam
cannot express them: one needs shell:true for npm.cmd on Windows, one
redirects stdout to a real fd. Both are the rule's own documented second
option, not an escape from it.

Closure verified rather than asserted: the derivation scan reports 0 unbounded
spawn helpers and 0 unbounded direct git call sites, and a temporary file
carrying an unbounded spawn still errors with the allowlist gone.

Closes #3064.

* test(#3148): close a hole in the guard's own eslint-disable ban

The ban listed only the top level of tests/, so it was blind to 37 .cjs
files under tests/helpers, qa, observability, fixtures and dispatch. With the
allowlist deleted this test is the sole remaining way to detect someone
silencing the rule inline, so the gap was load-bearing: a nested file could
carry an unbounded spawn plus an eslint-disable and pass everything.

Proven before and after. A probe planted under tests/helpers with both was
invisible to the guard and clean under eslint; after making the listing
recursive the guard fails on it. The scanned set goes from 771 files to 808.

Pre-existing since the guard shipped, but this wave is what promoted it to
sole defense, so it is fixed here rather than filed.

Also converts the last hand-rolled throw check to throwIfFailed and the last
re-derived legacy shape to compose toLegacyResult, which makes the epic's
none-remain claim true rather than nearly true. toLegacyResult itself is not
widened -- eight callers depend on its shape and one consumer does not
justify changing a shared contract.

* fix(#3148): correct seam incoherence at the bound and a slow review-lane error path

Two real failures from the remote runner, both fixed at the cause.

The seam could return outcome TIMED_OUT together with exitCode 0. At the
exact bound spawnSync reports ETIMEDOUT while the child has already exited
with a real status, and toSeamResult classified on the error code while
passing status straight through -- an incoherent pair its own boundary test
was written to catch, and did. A status that is not null is direct evidence
the child exited on its own, so it now decides the outcome before the
error-code branches run. process-seam.cjs was deliberately untouched by every
earlier wave; this is a defect in the module itself, kept surgical, with a
unit test that fails against the old logic.

review-lane with an unknown subcommand fell through to its usage error only
after loading the capability registry and building a per-lane plan, which
spawns one child process per lane -- up to twelve. The error path took
~1288ms instead of ~119ms, and under bench load it outran a caller's spawn
timeout and was killed before writing anything, which is the empty stdout and
stderr CI saw. It now fails fast before any of that work begins.

This is the epic's first production change. It is user-facing, so it carries
a changeset rather than a no-changelog label.

* test(#3148): replace a real-race timeout test with a deterministic one

E9 raced git rev-parse against a 1ms bound and assumed git always lost. On a
warm container git finishes first, spawnSync returns status 0 with no error
at all, the seam correctly classifies EXITED, and gitOrThrow correctly does
not throw -- so the test failed on both lanes. A probe confirms a genuine
timeout always carries status null, so this was never the seam misbehaving.

Raising the bound would only lengthen the odds, which is the same defect with
better luck. The test now drives gitOrThrow against a stubbed runGit that
returns a synthetic TIMED_OUT result, so it asserts exactly what it always
meant to -- that a timeout propagates as a throw -- with no timing
dependence. Five consecutive runs are identical where the old one varied.

I wrote this test in Wave 0; it is a real-race test by construction and
CLAUDE.md says to replace those rather than re-run them.

only, no switch). The commit always lands on the current branch. Callers
that want to be on the phase branch use execute-phase's handle_branching.

Updated 3 existing tests that asserted the old switch behavior.

src/context-predicates.cts (parser + selector + index builder) compiled to
gsd-core/bin/lib/, plus scripts/gen-context-index.cjs following the repo's
--check/--write drift-guard idiom and wired into lint:generated-sync.

Parser behavior is deliberately prototype-equivalent in this commit so the
next commit's regression matrix binds to the real defects rather than to a
missing module.

Two locked design deviations from the prototype:
- duplicates carry a count, not line numbers
- the committed index carries no line field at all, resolving ADR-1671 open
  question 4: an artifact without line numbers cannot drift on a line shift,
  so promoting --check to a CI gate does not make it routinely red

Also reconciles the one remaining duplicate predicate ID
(RULESET.WORKFLOW_MARKDOWN.FENCES was declared twice; the non-MD040 wording
is removed) so the gate can land fail-closed on duplicates.

Adds the regression matrix from the phase test plan: parser declaration
forms, fence and comment regions, ID/value grammar boundaries at
limit-1/limit/limit+1, CRLF fidelity, duplicate detection, the drift-guard
CLI, the selector query surface, and four document-shaped fast-check
properties.

Seven rows are RED for behavioral reasons against the ported parser:
indented-bare, star-list, plus-list and numbered-list declaration forms are
dropped; a tilde fence and a four-backtick fence containing a shorter fence
are not skipped; and a multi-line HTML comment is parsed as live. Eleven
selector rows are RED because the query surface is not wired yet.

Negative fixtures come from real repo documents that predate the grammar
(CONTEXT.md, CONTRIBUTING.md's fenced env-assignment examples) per the
fixture-provenance rule, and the property generators are document-shaped
rather than seeded from our own serializer.

Parser: replaces the ported naive triple-backtick toggle with the shared
markdown-sectionizer fence engine. scanFencedBlocks and FencedBlockRecord
gain an export keyword — the only change to that module, which has 71
upstream dependents — because it already returns line-indexed spans, which
is exactly what a line-reporting parser needs. It also already documents
itself as the second copy of the fence state machine pending consolidation;
adding a third copy here would have been the generative-fix divergence this
repo warns about. A parity suite now pins predicate fence-skipping against
that scanner across eight fence shapes. HTML-comment skipping stays local
because the sectionizer has no comment scanner. Declaration forms widen to
indented-bare, star, plus and numbered list items.

Index location: docs/CONTEXT-INDEX.json, not a module under bin/lib. The
remote matrix run caught the original choice — a committed .cjs there ships
~120KB of CONTEXT.md prose into a runtime module, and two content guards
fired truthfully on it (a leaked .claude install path, and four hardcoded
package-name literals). Neither guard was allowlisted; the artifact moved
instead, mirroring docs/INVENTORY-MANIFEST.json. Nothing at runtime needs to
require it — it is a drift-detection artifact, so the selector parses
CONTEXT.md live and is always current.

Generator: adds a frozen REASON enum and --check --json so the gate's
outcome is asserted structurally instead of by matching prose, and
--context-path/--index-path so tests drive the real CLI against a temp tree
with no filesystem monkeypatching.

Selector: gsd_run query context-predicates with --class/--prefix/--contains,
structured output carrying a matched count, own-property guards, and no
project-root resolution. Registering it exposed that the query dispatch
table and the usage string had drifted: a new parity test found 20 routed
commands missing from the usage list, all added here rather than deferred.

Exporting scanFencedBlocks made it public API for the first time, so it
needs its own contract test independent of the consumer that motivated the
export. Memtrace's co-change analysis flagged the gap: this suite changes
together with markdown-sectionizer.cts 8 times in 90 days and was absent
from the diff.

Covers the documented rules: 0-based indices, -1 for an unterminated fence,
the same-char/>=length/no-trailing-text closer rule, a shorter fence inside
a longer one staying content, CommonMark 4.5 backtick-in-info-string, and
<=3-space indent tolerance.

Two independent reviewers (correctness axis and security axis, neither the
author) found seven findings. All are fixed here with regression tests; none
deferred.

BLOCKER — comment-blind fence scanning caused silent, permanent predicate
loss. The HTML-comment scan and the fence scan ran as two independent passes,
and the fence scanner is comment-blind, so a fence delimiter inside an HTML
comment with no later close read as an unterminated fence and skipped every
remaining line to EOF. Worse, the drift-guard could not catch it: it diffs
against a baseline produced by the same corrupted parse. The two constructs
now interleave in a single pass so each suppresses the other's boundary
detection while active, covered in both directions. The parity suite still
binds this scanner to markdown-sectionizer's for comment-free documents, so
the two cannot diverge unnoticed.

BLOCKER — the selector was not consumed anywhere, leaving the phase's
acceptance criterion unmet. Now wired into the pre-work predicate-citation
step in contributor-standards, which is the repo's actual brief-assembly
path; no code-level brief assembler exists to wire into.

MAJOR — ReDoS with an unauthenticated CI-hang exploit. The predicate-id
regex nested a dot-containing character class inside a dot-prefixed repeat,
so N consecutive dots had exponentially many partitions: 40 dots took 565ms
and growth was exponential. CI runs this parser over a pull request's own
CONTEXT.md, so any contributor could have hung a shared runner with one
line. Replaced with linear per-segment validation. Doubled-dot ids are now
rejected; the real document contains none.

MAJOR — the duplicate-id gate had only ever been proven on synthetic
fixtures. A test now re-inserts the exact line this branch removed and
asserts the real generator names it.

MAJOR — --check together with --write silently let write win, turning the
gate into a writer; a missing path value resolved to the cwd and leaked an
EISDIR stack trace. Both are now clean usage errors.

MINOR — the hoisted skip-list was exported as a live mutable Set; replaced
with a read-only predicate. MINOR — flag-shaped selector values were
unmatchable; the inline --flag=value form now provides the escape hatch.

cmdCommit auto-detected the commit's phase from --files with an unanchored
`match(/(\d+(?:\.\d+)*)-/)`, which returns the leftmost digit-run-then-hyphen
anywhere in the joined path. A project_code ending in a digit (PROJECT_V2) made
`.planning/phases/PROJECT_V2-07-name/…` match the `2-` inside `V2-` before the
real `07-` token, resolving phase 2. findPhaseInternal also searches archived
milestones, so an existing archived phase 2 produced a real branch name and the
silent `git checkout <existing-branch>` fallback switched the whole working tree
onto the wrong branch in the same call that then committed.

The extraction now anchors to the directory segment immediately under
`.planning/phases/` (or `.planning/milestones/<v>-phases/`) and runs it through
the existing project-code-aware extractPhaseToken helper — the single owner
shared by the other 6 call sites — rather than introducing a fourth independent
copy of phase-token-matching logic. The auto-switch keeps create-if-absent only
(the #1278 intent: ensure the branch exists before the first commit on it); it
no longer force-switches an already-checked-out working branch onto a different
existing branch.

Adds two regression fixtures: a digit-suffixed project_code + an archived phase
whose number collides with the trailing digit (the silent-wrong-branch case),
and a pre-existing phase branch that must not be silently switched onto.

* test(#2539): assert non-silent warning; hoist execFileSync; normalizePhaseName guard

Address orthogonal-review findings on the #2539 fix:

- Spec AC2 ('an auto-checkout mid-commit must never happen silently'): the
  no-switch path now writes a 'Warning: resolved phase branch X already
  exists; committing on Y instead' line to stderr when checkout -b fails
  because the branch already exists. The regression test captures stderr via
  spawnSync and asserts the warning, so neither direction of the branching
  resolution is silent.
- Spec AC3 ('reuse normalizePhaseName/extractPhaseToken/stripProjectCodePrefix'):
  the token-acceptance guard now runs the candidate token through
  normalizePhaseName and accepts it only when it normalizes to a numeric phase
  form, rather than the brittle 'token !== phaseDir && /\d/.test(token)' check
  that leaned on extractPhaseToken's undocumented dirName fallback.
- Standards (Duplicated Code): hoist execFileSync/spawnSync requires to the top
  of the 'commit command' describe block instead of inlining them per test.

* fix(#2539): build phase-token shape from PHASE_NUMBER_TOKEN_SOURCE (#2128 guard)

The acceptance guard regex in detectPhaseNumberFromFiles was a hardcoded
`/^\d+[A-Z]?(?:\.\d+)*$/i` — a literal re-derivation of the canonical
phase-number grammar, which the #2128 phase-id drift guard
(tests/phase-id-drift-guard.test.cjs) rejects unless sanctioned with a
`// phase-id-owner:` marker. Build it from the single-owner
PHASE_NUMBER_TOKEN_SOURCE export instead, so this read-side acceptance check
cannot drift from every other phase-token reader. gsd-test reported this as
2 failures (linux-node22 + linux-node24) on the prior commit.

lint-fix-has-regression-test.cjs: new gate that fails if a fix(#NNNN)
or feat(#NNNN) commit has zero behavioral test files (*.test.cjs,
excluding auto-generated fixtures/baselines) in its diff. Wired into
lint:ci so it runs before PR creation.

Missing regression tests added:
- #2429: codex local scope does not set $HOME/.agents skills home;
  global scope does (tests/runtime-artifact-layout.test.cjs)
- #2279: map-codebase instructions say to overwrite existing dates,

Two coupled bugs from #2408:

1. cmdStats last-write-wins status (src/commands.cts:1610-1618): when two
   on-disk phase directories normalize to the same phase key (e.g.
   `05-real/` and `05-real-stray/`), the directory-scan merge overwrote
   `status` with whatever the *current* directory in scan order computed,
   discarding `existing?.status` entirely. fs.readdirSync order is not
   stable across platforms, so /gsd-stats could silently report `Not
   Started` for a phase that is actually `Complete`. Plan/summary counts
   were already additively merged; only `status` was wrong.

   Fix: introduced a `foldPhaseStatus(a, b)` helper that returns whichever
   status is further along the precedence ladder
   `Complete > Needs Review > Executed > In Progress > Planned > Not Started`
   (with `Pending` and unrecognized statuses ranked after). The merge site
   now calls `existing ? foldPhaseStatus(existing.status, status) : status`.
   The fold is commutative, so the result is identical regardless of read
   order.

2. cmdValidateHealth had no collision-detection pass (src/verify.cts):
   codes W001-W022 cover every condition except normalized-key collisions,
   so an operator got zero signal that anything was wrong.

   Fix: added W023 — groups phaseDirEntries by their normalized phase key
   (via the existing normalizePhaseName + extractPhaseToken helpers from
   phase-id.cjs — same normalization cmdStats uses) and emits a warning
   for any group with ≥2 dirs. The warning names the normalized key, both
   directory names (sorted by comparePhaseNum for stable output), and each
   directory's independently-computed status (via determinePhaseStatus
   imported from commands.cjs). Wording is deliberately neutral — never
   guesses which directory is the real one. The optional --repair path
   from the issue is intentionally NOT implemented in this PR (the issue
   marked it lower priority and acceptance criterion 4 is vacuously
   satisfied by omission).

Triage correction applied: the issue proposed W022, but that code is
already in use for config.json model-tier validation (src/verify.cts
:1372-1391). The next free code is W023, used here.

Tests:
- tests/commands.test.cjs: integration test that 05-real/ (Complete) +
  05-real-stray/ (empty/Not Started) collide and stats reports the merged
  phase as Complete regardless of read order; plus a direct unit test of
  foldPhaseStatus asserting commutativity + correct precedence for every
  status pair + correct handling of unrecognized statuses.
- tests/health-validation.test.cjs: integration test that W023 fires on
  the collision naming both dirs + their statuses (and uses neutral
  wording), plus a negative test that no W023 fires when only one dir
  exists per key.

References: #2408; reporter's three-layer triage + acceptance criteria;
triage correction that W022 is already in use (model-tier validation).

Each case body was relocated verbatim to a module-scope route*Command
function via a brace-matching extractor; inner break; statements (from
_dispatchNonFamily early-exit patterns) were converted to return;
(5 arms affected); loop break; statements preserved.

Tier mapping is 1:1 (Sol↔flagship, Terra↔balanced, Luna↔fast),

85ed50cc4f test(#1972): consolidate 94 command/module regression tests into subject suites
Fold 94 issue-named command/module regression files into the canonical test file
that owns each subject-under-test, across 52 existing suites (state, config, frontmatter,
roadmap-parser, capability-registry, shell-command-projection-dispatch, plan-phase-drift-guard,
health-validation, runtime-converters, commands, etc.). Verbatim block-scoped describe
wrappers; 881 subtests conserved 1:1. No new test files.

Host-env pre-check (per B2): the only GSD_WORKSTREAM/GSD_PROJECT-touching destinations
(intel, planning-workspace) clear those vars hermetically, so folded CLI tests are safe.

Regenerates regression-name allowlist (222->162), ratchets file-count allowlist across
8 buckets (validate entry removed after dropping <=2), makes 34 relocated allow-test-rule
exemptions issue-ref-compliant (ADR-456; prunes 34 stale ids). Repoints CONTEXT.md +
ADR-0002/443/1235/3524 test-file references. lint:ci green.

Part of epic #1969. Closes #1972.

- Scans each repo with `git -C "$REPO" status --porcelain`

- For selected repos: creates a PR branch, commits all staged/unstaged
  changes, pushes, and opens a companion PR via `gh pr create`

All git commands use `git -C "$REPO"` — never `cd "$REPO"` — because
shell state does not persist between agent-executed commands.

  bash `continue`, but the per-sub-repo iteration is prose-driven (the agent
  loops, not a literal `for`), so `continue` would warn and no-op. Reframed as
  prose-gated control flow keyed on $SUBREPO_EXIT — no bash loop assumption.
- Windows portability: the new symlink security case now degrades gracefully
  (try/catch around fs.symlinkSync; skip just the symlink assertion when symlink
  creation lacks privileges) so it doesn't hard-fail on Windows CI.

Verified: seam exits 1 on error / 0 on success (error() → process.exit(1),
propagated through the shim), so the $SUBREPO_EXIT check is meaningful; bash -n
clean on the touched blocks; commands 156/156; lint:ci green; manifest in sync.

`query user-story.validate` was a phantom command invoked by mvp-phase.md
(line 102) and verify-work.md (line 170) but had no CJS handler. Every
call exited with "Unknown command: user-story". The dotted-form dispatcher
strips the `query` prefix, splits on `.`, yielding command='user-story'
which fell through to the `default:` case with no registered capability
handler.

Adds a `case 'user-story':` handler inline in gsd-tools.cjs (same pattern
as the #1140 fix in PR #1148). The handler:

- Validates "As a [role], I want to [capability], so that [outcome]."
- Uses \S anchors to require non-whitespace content in each slot
  (whitespace-only slots like "As a  , I want to ..." now correctly
  return valid:false — found by adversarial Codex review)
- Returns { valid: boolean, errors: string[], slots: {role, capability,
  outcome} | null }
- Supports --pick valid for bare boolean output (verify-work.md usage)
- Added 'user-story' to SKIP_ROOT_RESOLUTION (pure string validation)
- Added 'user-story' to TOP_LEVEL_USAGE command list

Regression tests added to tests/commands.test.cjs (per lint-regression-

1d90ad3c30 fix(commands): route nested sub_repos by longest prefix, not array order (#1130)
groupFilesBySubrepo selected the first sub_repos entry in array order
whose prefix matched a file, so a file under a more-specific nested
sub-repo (e.g. packages/core/widget.js with sub_repos
["packages", "packages/core"]) was mis-routed to the less-specific
parent ("packages").

Select the longest (most-specific) matching prefix within each
first-segment bucket instead, making routing independent of sub_repos
array order. String()-guard the length comparison so non-string entries
still never throw (preserves the #311 tolerance). Update the stale doc
comment that claimed first-match semantics.

Closes #391.

cmdSummaryExtract read only the kebab frontmatter key
`requirements-completed`, but the tool's own JSON output key and the
milestone-audit `--pick` both use the snake form `requirements_completed`.
A SUMMARY written in the snake form the tool itself emits was silently
read back as [] — a false negative in milestone requirement traceability
with no diagnostic.

Make the reader tolerant of both key forms (kebab takes precedence), so a
round-tripped field is no longer dropped. extractFrontmatter does no
hyphen<->underscore normalization, so distinct keys had to be read
explicitly.

Adds regression tests: snake-only fixture now returns the IDs, and a
both-forms-present fixture asserts kebab precedence.

Renames the installed runtime directory `get-shit-done/` to `gsd-core/` so the
on-disk name matches the package (`@opengsd/gsd-core`), repo, and binary
(`gsd-tools`). The npm package name and binary are unchanged; npx/npm consumers
are unaffected.

Mechanical (bulk, ~90% of the diff):
- `git mv get-shit-done gsd-core`
- Swept path/identifier references across the repo via
  `perl -pe 's/get-shit-done(?!-\w)/gsd-core/g'`. The negative lookahead
  preserves the five legitimate slug variants that are NOT the directory:
  get-shit-done-{OLD,cc,classic,cli,redux} (old package/repo names).
- Build/manifest wiring: package.json (bin, files, coverage globs),
  tsconfig.build.json (outDir), ~86 .gitignore build-output entries,
  stryker.config.mjs, scan-ignore files, install.js path strings.
- Frozen (not rewritten): CHANGELOG.md history; translated docs
  (README.<locale>.md and docs/{ja-JP,ko-KR,pt-BR,zh-CN}/).

New logic (review here):
- src/installer-migrations/003-rename-get-shit-done-to-gsd-core.cts: a proper
  ADR-0008 installer migration. On upgrade it walks the legacy
  `~/.claude/get-shit-done/` tree, classifies each file via the prior install
  manifest, and emits remove-managed / backup-and-remove for managed files
  while PRESERVING unknown user-added files. Symlink-safe (skips a symlinked
  root and symlinked entries; bounds-checks every path under configDir). The
  framework rolls back on install failure. Emptied dirs may remain (framework
  has no recursive dir-removal primitive) — documented.
- scripts/lint-legacy-dir-name.cjs: CI regression guard forbidding the bare
  `get-shit-done` directory token (split token to avoid self-match; case-
  insensitive; `(?!-\w)` lookahead allows the slug variants; allowlists
  CHANGELOG, translated docs, and `gsd-allow-legacy-name` marker lines).
  Wired into the lint-tests CI job.
- Restored scripts/lint-package-identity-drift.cjs detection regexes (the
  mechanical sweep had wrongly rewritten the old-name patterns it exists to
  detect) and marked them as intentional legacy references.
- TDD tests for the migration and the guard; do.md slash-command guard regex
  tightened so a `/gsd-core/bin` path segment is not mistaken for a command;
  changeset + docs/installer-migrations.md row added.

Breaking: the installed runtime path moves `~/.claude/get-shit-done/` ->
`~/.claude/gsd-core/`. Migration 003 removes the stale legacy dir's managed
files (preserving user files) on upgrade. Users with custom hooks/configs
hardcoding the old path must update them.

alphabetically before runtime-launcher-parity.test.cjs in the same node
--test invocation. Each runs in a separate worker process but shares the
same HOME. The drift test's install leaks gsd-tools.cjs into that HOME,
then the launcher test's bash subprocess finds it via the $HOME/.claude arm.

Fix: add process.env.GSD_TEST_MODE = '1' at the top of the drift-guard
test, before the require(installPath) call. This matches the pattern used
by feat-443-effort-fast-mode.test.cjs and feat-443-effort-install-wiring
.install.test.cjs.

bd98e568f6 fix(#308): bound websearch fetch with timeout and retry (#387)
cmdWebsearch called fetch() with no timeout and no retry, so a hung
connection blocked indefinitely and transient 429/5xx/network failures
were not recovered. Add AbortSignal.timeout (configurable via
GSD_WEBSEARCH_TIMEOUT_MS, default 10s) and a bounded retry loop
(max 2 retries, exponential backoff + jitter) for 429/5xx/network
errors, honoring Retry-After on 429 (capped at 60s). Non-429 4xx fail
immediately (no wasted retries). Transient-exhausted failures report an
`attempts` count. Worst-case time is bounded by timeout*(1+retries)+backoff.

cmdCommitToSubrepo routed each changed file to a sub-repo via
subRepos.find(...) inside the file loop — O(files * repos). Extract a pure
groupFilesBySubrepo() that buckets sub-repos by first path segment and scans
only the matching bucket, dropping it to expected O(files + repos). First-
match-in-array-order semantics (incl. multi-segment sub-repos) are preserved
exactly. Adds the first behavior-lock test for the routing path.

c21bbf116e fix(tests): use array form for space-containing paths in CLI invocations
On machines where os.tmpdir() returns a path with a space (e.g.
/Volumes/Mini Me/tmp), runGsdTools() string args were whitespace-split by
the helper tokeniser, truncating paths at the first space.  Switch all
calls that embed a dynamic path into the argument list to the array form
of runGsdTools() so execFileSync receives each path as a single argv slot.

The base lint (scripts/lint-no-source-grep.cjs) only catches
readFileSync(...).<text-method>() chained directly. The much more
common var-binding form escapes it:

Scan of the test suite found ~141 files using this pattern.

Implementation built TDD per #2982 with structured-IR assertions:

  scripts/lint-no-source-grep-extras.cjs
    - detectVarBindingViolations(src) — pure detector, two passes:
      pass 1 collects vars bound from readFileSync, pass 2 finds any
      <var>.<includes|startsWith|endsWith|match|search>( on those vars.
    - detectWrappedAssertOkMatch(src) — flags
      assert.ok(<expr>.match(...)) which escapes the assert.match rule.
    - VIOLATION enum exposes stable codes for tests to assert on.

  scripts/lint-no-source-grep.cjs
    - Wires the new detectors into the existing per-file check; one
      additional violation row per file with the first 3 sample tokens.

  tests/bug-2982-lint-var-binding.test.cjs
    - 13 tests, all assertions on typed VIOLATION enum / structured
      records. Covers all 5 text-match methods, multi-var, no-bind,
      string literal (must NOT trigger), wrapped assert.ok(.match),
      and assert.match (must NOT double-flag).

Migration backlog (#2974 expanded scope):

  - 42 files annotated `// allow-test-rule: source-text-is-the-product`
    (legitimate — they read .md/.json/.yml files whose deployed text
    IS the product)
  - 3 files annotated `// allow-test-rule: pending-migration-to-typed-ir [#2974]`
    (read .cjs/.js source — clear migration debt)
  - 95 files annotated `pending-migration-to-typed-ir [#2974]` with
    `Per-file review may reclassify as source-text-is-the-product
    during migration` (mixed — manual review under #2974)

After this lands the lint reports 0 violations on main; new
violations in PRs surface immediately.

7b0a8b6237 fix: normalize phase numbers in stats Map to prevent duplicate rows (#2220)
When ROADMAP.md uses unpadded phase numbers (e.g. "Phase 1:") and
the phases/ directory uses zero-padded names (e.g. "01-auth"), the
phasesByNumber Map held two separate entries — one keyed "1" from the
ROADMAP heading scan and one keyed "01" from the directory scan —
doubling phases_total in /gsd-stats output.

Apply normalizePhaseName() to all Map keys in both the ROADMAP heading
scan and the directory scan so the two code paths always produce the
same canonical key and merge into a single entry.

Phases with all summaries but no passing VERIFICATION.md now show as
"Executed" instead of "Complete", preventing false progress reporting.

Adds determinePhaseStatus() helper used by both cmdStats() and
cmdProgressRender(). Also fixes duplicate phase directory accumulation
in cmdStats() — plans/summaries from directories sharing the same
phase number are now summed instead of silently overwritten.

New statuses: Executed (summaries done, no verification), Needs Review
(verification exists with human_needed status).

9ddd6c1bdc fix: create strategy branch before first commit, not at execute-phase (#1278)
When branching_strategy is "phase" or "milestone", the branch was only
created during execute-phase — but discuss-phase, plan-phase, and
new-milestone all commit artifacts before that, landing them on main.

Move branch creation into cmdCommit() so the strategy branch is created
at the first commit point in any workflow. execute-phase's existing
handle_branching step becomes a harmless no-op (checkout existing branch).

Also fixes websearch test mocks broken by #1276 (fs.writeSync change).

Add a cross_reference_todos step to discuss-phase that surfaces relevant
backlog items before scope-setting decisions are made.

Implementation:
- New 'todo match-phase <N>' CLI command (commands.cjs) that scores
  pending todos against a phase's ROADMAP goal using three heuristics:
  keyword overlap, area match, and file path overlap
- New cross_reference_todos step in discuss-phase.md between
  load_prior_context and scout_codebase
- CONTEXT.md template gains 'Folded Todos' subsection in <decisions>
  and 'Reviewed Todos (not folded)' subsection in <deferred>

Design:
- No AI call for matching — pure keyword/area/file heuristics for speed
- Silent skip when todo_count is 0 or no matches (no workflow slowdown)
- Auto mode folds all todos with score >= 0.4 automatically
- Scoring: keywords (up to 0.6), area match (0.3), file overlap (0.4)

Tests: 5 new tests covering empty state, keyword matching, unrelated
todo exclusion, area matching, and score sorting.
