* fix(#1257): update pipe-table Status/Phase/Plan cells in planned-phase + begin-phase cmdStatePlannedPhase ran its body-field replacements on the full file content, so the case-insensitive ^Status: pattern matched the YAML frontmatter `status:` line before the body `| Status | … |` cell — the cell never advanced to 'Ready to execute' and syncStateFrontmatter re-derived the stale 'planning' status (the #1230 delta heuristic preserved it). cmdStateBeginPhase had pipe-table else-branches only for Status/Last activity (#1256), so the Current Position `| Phase |` / `| Plan |` cells were left stale while a spurious inline `Phase: N — EXECUTING` line was prepended. Both handlers now strip frontmatter before body-field replacement and update the pipe-table cells in place via stateReplaceField, matching inline-format behaviour. Systemic residual of #1255 / #1256. Adds 4 regression tests (#1257 block in tests/state.test.cjs) covering both findings — RED before the fix, GREEN after; full state-area suite (437) and local unit suite stay green.
