* fix(#1577): address review — honest blocking docs, config key, ADR, property test, revert localized
- A1: rewrote the opt-in-blocking doc + Security changeset honestly — the PostToolUse hook is a
circuit-breaker (halts the agent's next step), NOT a redactor; it does not scrub content already
in the transcript. The prompt-level data/instruction boundary is the primary control.
- A2: registered security.injection_blocking in the config schema + defaults manifests (default
false) + an e2e config-roundtrip test; the dotted setter writes the nested shape the hook reads.
- A3: reverted the 4 hand-edited localized security-model.md (canonical EN only, per convention).
- A5: ADR-1577 (untrusted-input boundary + opt-in blocking; redaction-vs-circuit-breaker rationale).

* fix(#1577): allowlist untrusted-input-boundary.md in injection-scan CI gate
The new reference quotes injection phrases ('ignore previous instructions',
'you are now…') as examples agents must NOT comply with, tripping the repo's
own prompt-injection-scan.sh diff gate (the standalone 'security' CI job, red
on HEAD). Allowlist it alongside the other security docs (security-model.md,
TEST-EXAMPLES.md) that legitimately demonstrate injection patterns. The JS
scanner test doesn't scan references/, so only the shell gate needed it.
Verified: scan --diff origin/next -> 0 findings; scanner JS test 15/15.
* fix(#1577): cover AC #2's gsd-ui-researcher + gsd-assumptions-analyzer
trek-e Major 1: the @-included set dropped two AC #2 agents. Restore them so
no named web-ingress agent is uncovered, keeping the two justified additions
(gsd-ai-researcher, gsd-domain-researcher). Final set = AC's 8 + 2 = 10.
- gsd-ui-researcher carries the full WebSearch/WebFetch + MCP-fetch toolset.
- gsd-assumptions-analyzer reads 5-15 codebase source files (external/source-
document ingress per the boundary), though it has no web tools.
INGEST_AGENTS in the isolation test now asserts all 10; size baselines
regenerated (+60 bytes each, both well under the DEFAULT cap); changeset
reworded 8 -> 10.
Verified: untrusted-input-isolation 14/14; agent-size-budget 39/39.

The runtime launcher is not defined until the update_roadmap_and_state step
bash block (~line 165). The early verify_completion block used gsd_run to
query verification.status, which violated the runtime-launcher-parity test:
'preamble appears AFTER the first gsd_run reference'.
Replace the gsd_run call with an awk-based frontmatter extractor that reads
only the status: field between the two --- fences. This avoids both the
preamble-ordering constraint and the original false-positive grep bug where
body text like 'previous_status: gaps_found' would match a full-text regex.
The phase.complete gate at update_roadmap_and_state is the canonical
enforcement point; this early check is advisory only.
Also update workflow-size-baseline.json for the updated transition.md size.

for parity with readVerificationStatus's no-throw contract and testability. Also adds the

* fix(#1634): honor capability hook matcher and node-prefix command
Capability hook install (applyCapabilitySharedEdits) wrote each settings.json
hook entry with no `matcher`, so a tool-scoped hook fired on every tool (a
fail-closed guard could then block the whole session), and emitted a bare
single-quoted script path so a .js-family hook from a git/tarball source
without +x failed with Permission denied on every matching call.
- Pass through an optional declared `matcher` (entry-level sibling of `hooks`);
absent => omitted (match-all), so existing shipped capabilities are unchanged.
- Validate `matcher` in the declaration (non-empty string, no control chars).
- Emit `node <quoted-path>` for .js/.cjs/.mjs hooks (mirrors first-party);
.sh and others keep the bare quoted path (unchanged).
Root cause: the manifest hook schema (validator rule C4) was {event, script}
only with no matcher, and applyCapabilitySharedEdits never read or wrote one;
the command used shellSingleQuote(absScript) with no node prefix.
Regression tests fail-first on both defects (matcher dropped; bare path) and
pass after the fix; #1460 command assertions updated for the node prefix.

- validator: replace the control-character range regex with a char-code loop.
The literal /[\x00-\x1f\x7f]/ tripped ESLint's no-control-regex rule; char
codes are equally precise and lint-clean. Behavior unchanged (still rejects
matchers containing ASCII control characters incl. DEL).
- test: gate the executable-bit precondition on POSIX. Windows fs does not
honor POSIX write modes (a 0o644 write reads back as 0o666), so the
precondition is meaningless there and failed the windows-latest lane. The
node-prefix assertion — the actual fix — is platform-independent and still
runs everywhere.
* docs(#1634): amend ADR-894 for optional lifecycle hook matcher
The `role: "feature"` `hooks[]` entry now carries an optional `matcher`
(settings.json tool-scoping pattern: exact/pipe/wildcard/regex). Document
the field in the §2 schema table and record a Grilling-amendments entry:
the install path projects a declared matcher onto the emitted settings.json
hook entry (absent = match-all, so shipped capabilities are unchanged), and
per-runtime matcher projection (ADR-857 D8) stays a separate concern. This
amendment ships with the fix that introduced the field rather than as a
follow-up.
* docs(#1634): record WINDOWS-POSIX-MODE-BIT-ASSERT defect in CONTEXT.md
Capture the CI failure pattern from #1634/PR #1638 so it is not repeated: a
test that writes a file with a POSIX mode and then asserts statSync().mode
& 0o777 === <octal> passes on macOS/Linux but fails on windows-latest
(Windows fs does not honor POSIX write modes — reads back 0o666). Added as a
machine-greppable DEFECT predicate (symptom/examples/detect/fix-forward/
prevention) next to DEFECT.WINDOWS-TEST-PORTABILITY, with the fix-forward:
gate the mode-bit precondition on process.platform !== 'win32' and keep the
platform-independent behavioral assertion running everywhere.
PR #1622 (issue #1615) shipped Windsurf /gsd-* workflow wrappers that delegate to command bodies at <targetDir>/.windsurf/gsd-core/commands/gsd/X.md via a hardcoded @~/.claude/gsd-core/commands/gsd/ path. The path-rewrite pipeline correctly substitutes ~/.claude/ to the install target. But the source gsd-core/ dir does not ship with commands/ — the canonical command source lives at the package root (commands/gsd/). Without this copy, every /gsd-* workflow in Cascade references a file that does not exist. The slash commands appear in the / menu but silently fail when invoked because the LLM is told to read a missing file.
None of the original reviews caught this: not the security review, not Codex's adversarial orthogonal review (gpt-5.5/high), not Memtrace's graph-backed review. It was surfaced by a #1629 regression test that verifies 'every workflow @-reference target exists on disk after install' — the test failed, revealing the bug.
Fix: for Windsurf local installs, copy commands/gsd/*.md into <targetDir>/gsd-core/commands/gsd/ via copyWithPathReplacement (applies the same path+brand rewrites as the rest of the install). Guarded on isWindsurf && !isGlobal since global Windsurf workflow install is an explicit no-op.
Documented as DEFECT.WORKFLOW-DELEGATION-TARGET-NOT-INSTALLED in CONTEXT.md so the pattern is locked in: any new converter emitting a wrapper that delegates to another file MUST verify the delegation target is actually installed.
The commandName validation tests legitimately contain real injection payloads (newline + system-role override phrases, fake [SYSTEM] tags, jailbreak strings) to prove the validator rejects them. The scanner cannot distinguish a test fixture asserting rejection from an actual injection attempt, so CI failed on the test that adds the security control.
Added tests/windsurf-conversion.test.cjs to scripts/prompt-injection-scan.sh ALLOWLIST with a comment citing the defect class.
Also added DEFECT.PROMPT-INJECTION-SCAN-COLLISION-WITH-TESTS to CONTEXT.md so the pattern is documented. Initial draft of that predicate ITSELF triggered the scanner (it quoted the literal injection phrase as an example) — reworded to use descriptive references ('scanner-matching payload', 'instruction-override phrase') since the scanner scans CONTEXT.md too. That meta-collision is now called out in the fix-forward and prevention subkeys.
computePathPrefix returned a Windows-style path (with backslashes from path.join) into markdown @-references. Workflow file content on Windows ended up with mixed separators, breaking substring checks in install/install-runtime-artifacts tests on windows-latest CI only.
Normalize resolvedTarget and homeDir to forward slashes inside computePathPrefix. The prefix is always substituted into markdown body text, which uses POSIX paths universally. Idempotent on POSIX.
Also normalizes the two test assertions to forward-slash form so they pass on Windows. Adds a regression test for backslash-style input.
Documents DEFECT.WINDOWS-PATH-LEAK-IN-MARKDOWN-CONTENT + RULESET.CONTENT-PATH-NORMALIZATION in CONTEXT.md so this anti-pattern stops recurring.
Add an optional structured `coverage:` block to SUMMARY.md frontmatter and a
deterministic classifier that `verify-work` consumes to route deliverables to
auto-pass vs human-UAT — replacing the rejected #1598/#1599 post-hoc heuristic.
- New `src/coverage.cts` (→ bin/lib/coverage.cjs) parses the nested coverage
block (extractFrontmatter can't — its `-` items are scalars-only; this is a
focused parser, sibling of parseMustHavesBlock), validates each entry, and
classifies into auto_passed vs present. Frozen MODE/PRESENT_REASON/ERROR_CODE
typed-IR surface. Exposed via `uat classify-coverage --summary <f>`.
- Auto-pass is the narrow proven case only: strict-boolean human_judgment:false
AND non-empty all-`pass` verification AND zero validation errors. Everything
else — judgment, empty/failing verification, malformed entry — routes to the
human (fail-safe). A malformed block falls back to legacy prose extraction and
surfaces an error; an absent block is byte-identical to pre-#1602.
- execute-plan create_summary populates the block (fail-safe default
human_judgment:true); verify-work extract_tests consumes it; create_uat_file
marks auto-passed entries `source: automated`.
- Templates (summary + 3 variants), CONTEXT.md predicate + glossary, INVENTORY,
eslint/gitignore registration, and Diataxis docs (COMMANDS reference +
USER-GUIDE explanation) updated.
- Behavioral tests via the CLI (no source-grep); parser-robustness regressions
for the null-entry/comment-header/mis-indent cases found in adversarial review.

* fix(#1383): resolve GSD version without a top-level require of the runtime-root package.json
The extracted runtime-artifact-conversion module sits in the gsd-tools
loader chain and did a module-load `require('../../../package.json')`.
On Codex (whose runtime root has no package.json) that threw
`Cannot find module '../../../package.json'`, crashing every gsd-tools
command before it did anything. Even on Claude the synthetic
`{"type":"commonjs"}` has no `version`, so the sole consumer already
emitted `version: undefined`.
Resolve the version lazily and defensively instead: read the installed
gsd-core/VERSION, else lazily require the runtime-root package.json, else
degrade to '' so the caller omits the field. Both sources are validated
against the repo's semver-prefix convention (mirrors update-context.cts)
so a garbled VERSION is never emitted verbatim. install.js's dead
duplicate converter is intentionally left untouched (scoped to the crash).
Adds a #1383 regression block exercising resolveVersionFrom across
VERSION-only / package.json-only / neither / malformed-VERSION layouts,
asserting no-throw and the correct version string.

rebuilt expected paths without that normalization, so they passed on Mac/Linux
but failed on Windows CI (PR #1513):
- two absolute-branch asserts rebuilt resolvedTarget via path.resolve(configDir)
without the backslash→slash replace → mismatch on Windows.
- the $HOME-branch test fed a POSIX-literal /home/testuser, which Windows
path.resolve re-roots onto the cwd drive (D:/home/...), so the
resolvedTarget.startsWith(homeDir) check failed and the $HOME shorthand was
never produced.
Fix is test-only (engine unchanged, still behavior-preserving): mirror the
engine's .replace(/\\/g,'/') in the two absolute-branch asserts, and use a real
absolute path (path.resolve(os.tmpdir(), ...)) + platform: process.platform for
the $HOME-branch test so the comparison holds on all platforms.

Adds scripts/lint-resolution-provenance.cjs — a registry + ratchet CI guard
that locks in the agent-skills configured_empty/not_configured contract tests
so they cannot be silently removed, and establishes a registration point for
future config-interpreting read verbs (ADR-1411 P4).
Design rationale:
- REGISTRY (one entry: agent-skills → src/init.cts → tests/agent-skills.test.cjs)
is the canonical registration site; new verbs are added here.
- For each registered verb, the guard asserts its test file contains BOTH a
`configured_empty` assertion AND a `not_configured` assertion — proving the
configured-empty-vs-not-configured contract is explicitly tested.
- NOT a universal static detector (intractable / false positives) — mirrors the
no-adhoc-markdown-parsing grandfather pattern.
- Uses scripts/lib/allowlist-ratchet.cjs (assertWithinAllowlist) so stale
allowlist entries fail (ratchet-down) and novel offenders always fail.
- checkRegistry() is factored as a pure exported function tested in
tests/lint-resolution-provenance.test.cjs without shelling out.
- Wired into lint:ci (package.json) and lint step name updated in test.yml.
- CONTEXT.md ### Resolution Convention extended with P4 guard sentence.
- Allowlist starts empty ([]) — agent-skills already has its tests.

Narrows P3 of ADR-1411 (Resolution Provenance, epic #1411) based on an
adversarial fit-analysis that showed a single Resolution<T> envelope adopted
by agent-skills, capability-state, and capability-writer fails the deletion
test: configured/reason are meaningless for capability verbs, and
capability-writer's errors[] (operation-not-applied) cannot fold into
warnings[]. The only genuinely shared seam is warnings: string[].
Changes:
- src/resolution.cts: new pure types+builder leaf — exports Resolution<T>
{value, configured, reason, warnings}, makeResolution<T>() builder, and
AgentSkillsValue {block, skills_count}. No other src/ imports.
- src/init.cts: cmdAgentSkills --json IR gains additive value:{block,
skills_count} field (built via makeResolution). All existing flat fields
(agent_type, block, skills_count, warnings, configured, reason, source,
degraded) are retained unchanged for back-compat.
- src/capability-state.cts: doc comment on ResolveCapabilityRuntimeStateResult
naming it the canonical read-verb envelope. No JSON change.
- src/capability-writer.cts: doc comment on SetCapabilityStateResult naming it
the canonical mutation-verb result (warnings=advisory, errors=operation-
not-applied). No JSON change.
- CONTEXT.md: new ### Resolution Convention glossary entry after
### Resolution Provenance.
- docs/adr/1411-resolution-provenance.md: P3 narrowing amendment appended.

Add heuristic (4) to findProjectRoot: after heuristics (1)-(3) (sub_repos,
multiRepo, .git+parent-.planning/) are exhausted without a match, perform a
second bounded walk-up within FIND_PROJECT_ROOT_MAX_DEPTH to locate the
nearest ancestor directory containing a .planning/ subdirectory. Returns that
ancestor as the project root, so loadConfig finds the correct config instead
of falling through to defaults when gsd-tools is invoked from a plain
descendant subdirectory of a single-repo project (#1366 cwd-drift gap).
Ordering is load-bearing: the new walk runs AFTER the existing loop so
sub_repos workspaces (where a child sub-repo may have its own .planning/)
still resolve correctly to the parent workspace. The existing own-.planning/
guard (heuristic 0, #1362) and the depth bound (FIND_PROJECT_ROOT_MAX_DEPTH=10)
are both preserved unchanged.

Make src/capability-activation.cts the sole owner of the four-level config-key
precedence walk via a new raw-value primitive resolveConfigKey(dotKey,{config,
cwd,registry}); the boolean wrapper _resolveActivationValue and loop-resolver's
resolveConfigValues are both rebuilt on it. loop-resolver deletes its
byte-identical copy and inline re-walk and imports the engine.
resolveCapabilityRuntimeState no longer returns registry/config (leaked internal
detail); the caller loads one fail-closed config snapshot and threads it in via a
new optional configOverride param, so capability `active` and hook when/configValues
resolve against the same object. capability-writer requires the registry module
directly.
Adds a DEFECT.GENERATIVE-FIX parity gate (identity + behavioral matrix +
end-to-end resolveLoopHooks configValues) that fails if the two precedence
surfaces ever diverge. Pure internal refactor, no user-facing change.

shared isCapabilityActive('graphify', cwd) — so graphify is off unless installed

resolver: resolveCapabilityRuntimeState now detects the active runtime via
resolveRuntime(cwd) (GSD_RUNTIME -> config.runtime -> 'claude') so non-Claude
runtimes (Codex/Cursor) read their own surface, not ~/.claude. Hermetic
regression test proves config-on+unsurfaced -> disabled; cross-runtime test

Add an optional activationKey to the feature role of capability.json — the
dotted config key that gates the whole capability (e.g. graphify.enabled).
gen-capability-registry validates it (non-empty string, reserved-name guard,
must be declared in the capability's own config slice, feature-only) and emits
it per-capability in the generated registry. Declared on graphify + intel.
No runtime consumption yet (resolver wiring lands in #1305). Part of #1302.

Re-home the 6 implementation functions squatting in the core.cjs re-export
spine (ADR-857) into the modules whose interface they belong to, with core
re-exporting them BY REFERENCE so all 32 callers + the shim-identity tests
keep resolving unchanged:
- worktree-safety: resolveWorktreeRoot, pruneOrphanedWorktrees
- git-base-branch (broadened to the Git Query Module): gitWorktreeInfoInternal
- agent-install-check (new leaf): getAgentsDir, checkAgentsInstalled
- delete the _resetRuntimeWarningCacheForTests wrapper; consumers use a
shared resetRuntimeWarningCaches() helper in tests/helpers.cjs
Add scripts/lint-core-spine-imports.cjs (migration-convergence lint with a
30-importer allowlist, wired into lint:ci) so the staged spine retirement
provably converges: CI fails on any new ./core import. Register the new
generated agent-install-check.cjs in eslint-ignore + .gitignore +
INVENTORY-MANIFEST.json.

Edge-probe now surfaces a zero-classification requirement (non-empty prose, no
shape cue matched, no `shapes` override) as a single soft `unclassified — review
manually` candidate instead of silently dropping it — the exact blind spot the
probe exists to catch. Dismissible like any edge; the `shapes: []` opt-out stays
silent; `TAXONOMY` (the closed 8 categories) is unchanged. Under `--auto` the
candidate is left `unresolved`, never auto-`backstop` (a missing shape is not
evidence an edge exists).

* fix(#1146): single base-branch resolver across forking workflows
Replaces duplicated per-workflow bash detection that silently fell through
to :-main on repos where origin/HEAD is unset (git init+remote add+fetch
without set-head, most CI checkouts, many worktrees).
New CJS module git-base-branch.cjs exposes `gsd_run query git.base-branch`
with full precedence ladder: git.base_branch config override → origin/HEAD
symref → git remote show origin (authoritative) → local branch presence →
"main". All git subprocesses bounded with timeouts; degrades gracefully.
Wires execute-phase, quick, ship, complete-milestone, and pr-branch to the
single resolver. Removes 14 lines of duplicated detection bash across the
five workflows.
Includes 7 behavioral tests covering the full precedence ladder including the
key regression case (master repo, origin/HEAD unset → must return "master",
NOT "main") and an anti-regression guard that fails if any workflow
re-introduces the :-main/:-master fallback pattern.

inventory manifest. ship:post wired into ship.md (wire-on-demand).
HELD on #1196: this capability also declares hooks at discuss:pre and
discuss:post, which are structurally un-wireable until the host-loop
conformance model covers the discuss phase (discuss-phase.md is not in
HOST_LOOP_FILES). The phase6-capstone-conformance gate therefore fails
on exactly those two orphaned points by design — see #1196. Once #1196
lands, rebase onto next and the gate goes green with no further change.

scalars that collided silently on merge: two branches each bumping the
same integer to N+1 produced a clean git merge whose value the merged
filesystem (N+2) contradicted, hard-failing inventory-counts.test.cjs on
the CI merge commit across all platforms (DEFECT.INVENTORY-MERGE-UNDERCOUNT).
- Strip the six `(N shipped)` heading counts + the two prose footnote
counts; repoint the intro to INVENTORY-MANIFEST.json as the registry.
- Drop the decorative `generated` date from the manifest + its
strip-before-compare branch in gen-inventory-manifest.cjs (it conflicted
on cross-day merges and is read by nothing).
- Delete inventory-counts.test.cjs (scalar-vs-disk gate, the collision
source); its drift protection is subsumed by the merge-safe set-membership
test inventory-manifest-sync.test.cjs, which stays as the sole gate.
- Add inventory-headings-countfree.test.cjs guard (fails if a count is
re-added to a heading).
- Fix already-broken count-bearing cross-doc anchors to stable count-free
slugs in ARCHITECTURE.md + multi-agent-orchestration.md.
- Retire the now-impossible DEFECT.INVENTORY-MERGE-UNDERCOUNT + obsolete

The sandboxTier runtime-capability axis was cosmetic: declared and validated
on all 16 descriptors but read by nothing. The codex per-agent sandbox_mode
line was emitted unconditionally from the hardcoded CODEX_AGENT_SANDBOX map, so
the descriptor field drove no behaviour (ADR-857 audit finding F10; the
"rides along in 5e/5g" promise in ADR-1016 §8 never landed).
Make the axis load-bearing:
- resolveInstallPlan projects sandboxTier as a 7th InstallPlan axis and fails
loud (throws) on a missing/invalid value rather than coercing to 'none'.
- installCodexConfig / generateCodexAgentToml gate sandbox_mode emission on
sandboxTier !== 'none'.
- The per-agent CODEX_AGENT_SANDBOX map is kept: it is GSD agent policy, not a
runtime-descriptor property (different layer). Full removal of that map is
tracked under #1138 (phase-6 descriptor-residue removal).
For codex (sandboxTier === 'codex-agent-sandbox') the emitted TOML is
byte-identical to before; for 'none' runtimes sandbox_mode is omitted.
Adds leaf, projection, and installCodexConfig threading-seam regression tests;
updates the enh-1082 InstallPlan golden master with sandboxTier for all 16
runtimes. Confirmed hypothesis: schema-first vocabulary closure outran consumer
wiring, with no conformance gate to catch the orphaned axis.

Codify the audit search-discipline rule surfaced by the ADR-857 phase 1-5 review: verify whether an invariant/validation EXISTS by searching the AUTHORED source (src/*.cts or the scripts/gen-*.cjs generator), not the generated gsd-core/bin/lib/*.cjs artifact (gitignored per ADR-457). A generator's gen-time validation lives in the gen-*.cjs script, not the declaration it consumes; search both before declaring an invariant absent.
Prevents the false-negative that reported ADR-1016 5e's ConverterName enum as unenforced when VALID_CONVERTER_NAMES lives + is checked in gen-capability-registry.cjs.

The render-hooks resolver and capability-state resolver headers (and their CONTEXT.md glossary entries) still claimed "no workflow calls this yet" / "consumed by nothing yet (phase-6 wiring is out of scope)". Both are now false: loop.render-hooks is consumed live by plan-phase.md/autonomous.md (plan:pre ui-phase, verify:post ui-review), and capability-state is the `gsd-tools capability state` CLI diagnostic.
Comment/prose only — no behavior change. Surfaced by the ADR-857 phase 1-5 completeness audit.
