test(#1972): consolidate 94 command/module regression tests into subject suites
Fold 94 issue-named command/module regression files into the canonical test file that owns each subject-under-test, across 52 existing suites (state, config, frontmatter, roadmap-parser, capability-registry, shell-command-projection-dispatch, plan-phase-drift-guard, health-validation, runtime-converters, commands, etc.). Verbatim block-scoped describe wrappers; 881 subtests conserved 1:1. No new test files. Host-env pre-check (per B2): the only GSD_WORKSTREAM/GSD_PROJECT-touching destinations (intel, planning-workspace) clear those vars hermetically, so folded CLI tests are safe. Regenerates regression-name allowlist (222->162), ratchets file-count allowlist across 8 buckets (validate entry removed after dropping <=2), makes 34 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456; prunes 34 stale ids). test(#1971): consolidate 48 gsd-tools CLI regression tests into subcommand suites
Fold 48 issue-named gsd-tools CLI regression files into the canonical test file that owns each subcommand subject (state, roadmap, phase, milestone, audit, config, router/dispatch, stats, verify, health, etc.), preserving every assertion and its origin issue number as provenance (block-scoped describe wrappers, 299 subtests conserved 1:1). No monolithic gsd-tools.test.cjs created — routes into 18 existing per-subject suites. Removes 48 tests/ files. Regenerates regression-name allowlist (271->231), ratchets the file-count allowlist across 6 buckets (audit/milestone/phase/roadmap/state/verify), and makes 10 relocated allow-test-rule exemptions issue-ref-compliant (ADR-456; prunes 10 stale ids). docs(#1817): add adr-1817 state.md rebuild derivability contract (#1828)
* chore(context): scrub nul byte from consent-store predicate
CONTEXT.md line 206 (Capability Consent Store predicate) contained a
literal NUL byte between ${realpath(projectRoot)} and <id> documenting
the disk-key join format. The byte was intentional but made the file
binary-detected, breaking grep/rg searches (hit while preparing ADR-1817).
Replace with the 4-char \x00 escape. Preserves the byte-level disk-key
documentation; surrounding prose 'prototype-pollution-safe NUL-joined
keys' carries the semantic context. file(1) now reports 'Unicode text'.
* docs(#1817): add adr-1817 state.md rebuild derivability contract
Phase 0 of approved feature #1817 (epic). Lands the design contract for
the new `rebuild` transition in the STATE.md Transition Module (ADR-1769):
- ADR-1817 (new): `rebuild` is the capstone 11th transition. Six design
decisions: (1) core substrate, non-toggleable, same tier as the other
10; (2) section taxonomy — re-derivable (`## Current Position` prose
from frontmatter, `## By-Phase Progress` table from disk) vs preserved
(`## Session`, `## Decisions`, unknown sections) vs de-duplicated
(`## Session Continuity Archive`); (3) orphaned data is logged + dropped
with a structured audit entry in `## Rebuild Log` (ADR-1411 provenance
principle); (4) idempotency is a hard guarantee — a no-mutation rebuild
appends no log entry; (5) non-overlapping scope with `sync` (3
frontmatter fields, auto-triggered); (6) orthogonal to
`auto_prune_state` (rebuild reconciles with current canonical sources,
prune removes by retention policy).
- CONTEXT.md (STATE.md Transition Module section): list `rebuild` as the
11th intent; add contract predicates mirroring the ADR.
- docs/adr/README.md: add ADR-1817 to the index (Accepted).
Targets the #1776/#1761/#1591 body-drift cluster that survived ADR-1769's
per-field transitions. Phased per ADR-1817: this PR (Phase 0) closes
#1817; Phase 1 (#1827) lands `rebuildCore` + intent dispatch + drift-class
unit tests; Phase 2 (#1826) lands `cmdStateRebuild` CLI + dry-run +
integration tests + docs + changeset.
* docs(#1817): reword state-doctor alternative to satisfy docs-parity lint
The docs-parity-live-registry test scans every docs/*.md (including
docs/adr/) for slash-command tokens and asserts each one resolves to a
live command in the registry. The rejected-alternative #4 in ADR-1817
mentioned a hypothetical `/gsd:state-doctor` workflow, which tripped
the lint (`unknown command token(s): [/gsd:state-doctor]`).
Reword to 'standalone state-doctor workflow' (no slash prefix). The
extractor is aggressive — backticks and space-preceding tokens are both
extracted per the test's own polarity-invariant cases — so the only
sound fix is to not form a slash token at all for hypothetical names.
Verified locally: `node --test tests/docs-parity-live-registry.test.cjs`
now passes 31/31 (was 30/1).

Adds the second production-code portability AST rule + the ADR-mandated glob expansion to bin/install.js and scripts/build-hooks.js. - eslint-rules/require-fs-op-fallback.cjs: flags an unguarded fs.rename / fs.renameSync (the atomic-publish primitive named first in DEFECT.WINDOWS-FS-OPS.symptom) that is NOT inside a try/catch whose handler references a transient errno ('EPERM'/'EBUSY'/'EACCES' or a *RETRY_ERRNOS set) AND NOT behind a Windows platform guard. A catch that silently swallows or cleans-up-and-rethrows without an errno check does NOT satisfy the defect's 'never silently swallow' clause. copyFile/unlink are deliberately not flagged (they are the fallback primitives per the defect's own fix-forward). Scope narrowed to rename per Phase 5's precision discipline; documented on #1740. - src/shell-command-projection.cts: export retryRenameSync(from, to) — the drop-in bounded-retry helper over the existing atomicRenameWithRetry. - 27 bare fs.renameSync sites across 11 modules routed through retryRenameSync (capability-lifecycle/lock/source, installer-migrations, milestone, phase, planning-workspace, roadmap-upgrade, runtime-hooks-surface, state, workstream). Idempotent on POSIX; resilient to AV/indexer transient locks on Windows. - eslint.config.mjs: register rule at error on src/**/*.cts; new focused portability-rules block covering bin/install.js + scripts/build-hooks.js (ADR-1703 L124-126 glob expansion — both files are compliant: zero rename violations). - tests: 15-case RuleTester suite; portability-rule-disable-ban extended (PROTECTED_RULES + scans bin/install.js/build-hooks.js with shebang handling); ci-test-scope portability-lint selection rule. - CONTEXT.md DEFECT.WINDOWS-FS-OPS predicate rewritten to point at the rule; docs/contributing/cross-platform-portability-rules.md reference + how-to. Closes #1740 * chore(#1740): backfill changeset pr:1742 * fix(#1740): tighten require-fs-op-fallback precision (codex review HIGH-1/HIGH-2) Addresses two false-negative findings from the codex (gpt-5.5/high) adversarial review of PR #1742: HIGH-1 — a catch that REFERENCES a transient errno but only rethrows (no retry/fallback) was marked compliant. The DEFECT.WINDOWS-FS-OPS fix-forward requires retry, not just recognition. Fix: catchHandlerHasRetrySignal now requires a loop `continue` backedge OR a `return <call>` delegation; a bare rethrow is flagged. The misleading `/* retry logic */` valid test is replaced with a real retry loop, and the rethrow-only shape is added as invalid. HIGH-2 — the nested-try ancestor walk treated an OUTER errno-catch as protecting the rename even when an INNER catch intercepted/swallowed the error (the outer catch is unreachable). Fix: isInsideTransientErrnoTryCatch now stops at the NEAREST enclosing TryStatement WITH A CATCH HANDLER whose block contains the rename (try-finally is skipped — it doesn't catch); outer catches are no longer consulted. The unsound nested-try valid test is converted to invalid, and a try-finally-skipped valid case is added. RESCOPED from the original issue: the glossary entries (Probe Family / Probe Core / Edge Probe / Prohibition Probe / Verification Tier / Verification substrate) ALREADY landed (CONTEXT.md), so only the predicate block was net-new — the draft's premise that the glossary 'never landed' came from a grep that hit 'binary file matches' on a multibyte char. Research-derived N17/N18 numbers deliberately kept out of machine-canon (they live hedged in docs/design/verifier-reach.md). Closes #1608. |

* fix(#1634): honor capability hook matcher and node-prefix command Capability hook install (applyCapabilitySharedEdits) wrote each settings.json hook entry with no `matcher`, so a tool-scoped hook fired on every tool (a fail-closed guard could then block the whole session), and emitted a bare single-quoted script path so a .js-family hook from a git/tarball source without +x failed with Permission denied on every matching call. - Pass through an optional declared `matcher` (entry-level sibling of `hooks`); absent => omitted (match-all), so existing shipped capabilities are unchanged. - Validate `matcher` in the declaration (non-empty string, no control chars). - Emit `node <quoted-path>` for .js/.cjs/.mjs hooks (mirrors first-party); .sh and others keep the bare quoted path (unchanged). Root cause: the manifest hook schema (validator rule C4) was {event, script} only with no matcher, and applyCapabilitySharedEdits never read or wrote one; the command used shellSingleQuote(absScript) with no node prefix. Regression tests fail-first on both defects (matcher dropped; bare path) and pass after the fix; #1460 command assertions updated for the node prefix. * chore(#1634): backfill changeset pr:1638 * fix(#1634): resolve lint and windows CI failures - validator: replace the control-character range regex with a char-code loop. fix(#1629): copy Windsurf command bodies so workflow delegation targets exist
PR #1622 (issue #1615) shipped Windsurf /gsd-* workflow wrappers that delegate to command bodies at <targetDir>/.windsurf/gsd-core/commands/gsd/X.md via a hardcoded @~/.claude/gsd-core/commands/gsd/ path. The path-rewrite pipeline correctly substitutes ~/.claude/ to the install target. But the source gsd-core/ dir does not ship with commands/ — the canonical command source lives at the package root (commands/gsd/). Without this copy, every /gsd-* workflow in Cascade references a file that does not exist. The slash commands appear in the / menu but silently fail when invoked because the LLM is told to read a missing file. None of the original reviews caught this: not the security review, not Codex's adversarial orthogonal review (gpt-5.5/high), not Memtrace's graph-backed review. It was surfaced by a #1629 regression test that verifies 'every workflow @-reference target exists on disk after install' — the test failed, revealing the bug. Fix: for Windsurf local installs, copy commands/gsd/*.md into <targetDir>/gsd-core/commands/gsd/ via copyWithPathReplacement (applies the same path+brand rewrites as the rest of the install). Guarded on isWindsurf && !isGlobal since global Windsurf workflow install is an explicit no-op. Documented as DEFECT.WORKFLOW-DELEGATION-TARGET-NOT-INSTALLED in CONTEXT.md so the pattern is locked in: any new converter emitting a wrapper that delegates to another file MUST verify the delegation target is actually installed. |

fix(#1615): allowlist windsurf-conversion.test.cjs in prompt-injection-scan
The commandName validation tests legitimately contain real injection payloads (newline + system-role override phrases, fake [SYSTEM] tags, jailbreak strings) to prove the validator rejects them. The scanner cannot distinguish a test fixture asserting rejection from an actual injection attempt, so CI failed on the test that adds the security control.
Added tests/windsurf-conversion.test.cjs to scripts/prompt-injection-scan.sh ALLOWLIST with a comment citing the defect class.
Also added DEFECT.PROMPT-INJECTION-SCAN-COLLISION-WITH-TESTS to CONTEXT.md so the pattern is documented. Initial draft of that predicate ITSELF triggered the scanner (it quoted the literal injection phrase as an example) — reworded to use descriptive references ('scanner-matching payload', 'instruction-override phrase') since the scanner scans CONTEXT.md too. That meta-collision is now called out in the fix-forward and prevention subkeys.

Normalize resolvedTarget and homeDir to forward slashes inside computePathPrefix. The prefix is always substituted into markdown body text, which uses POSIX paths universally. Idempotent on POSIX. Also normalizes the two test assertions to forward-slash form so they pass on Windows. Adds a regression test for backslash-style input. * fix(#1383): resolve GSD version without a top-level require of the runtime-root package.json The extracted runtime-artifact-conversion module sits in the gsd-tools loader chain and did a module-load `require('../../../package.json')`. On Codex (whose runtime root has no package.json) that threw `Cannot find module '../../../package.json'`, crashing every gsd-tools command before it did anything. Even on Claude the synthetic `{"type":"commonjs"}` has no `version`, so the sole consumer already emitted `version: undefined`. Resolve the version lazily and defensively instead: read the installed gsd-core/VERSION, else lazily require the runtime-root package.json, else degrade to '' so the caller omits the field. Both sources are validated against the repo's semver-prefix convention (mirrors update-context.cts) so a garbled VERSION is never emitted verbatim. install.js's dead duplicate converter is intentionally left untouched (scoped to the crash). Adds a #1383 regression block exercising resolveVersionFrom across VERSION-only / package.json-only / neither / malformed-VERSION layouts, asserting no-throw and the correct version string. ADR-1244 Phase 5 (D7). dispatchOverlayCapabilityCommand in gsd-tools.cjs dispatches an installed third-party capability command family via loadRegistry({includeInstalled}), gated on a committed ledger entry (consent) and confined to the capability's install root (defaultRequireFromInstallRoot: bare-.cjs basename + realpath containment, rejects ../ traversal + symlink escape); same own-property/function/sync/ExitError guards as the first-party path. capability-loader records _overlay.commandRoots only for accepted overlay caps with a committed, structurally-valid ledger entry (fail closed). First-party graphify/intel/audit unchanged (already on the registry seam). 3 Codex rounds converged + /security-review (no HIGH) + /code-review (Approve); gsd-test green both platforms; CI green.

Regression from #217 — pre-#217 returned ~/.gemini/antigravity unconditionally. Add an optional probeMarker (gsd-core/VERSION) to the dot-home-nested descriptor: a two-pass probe prefers the candidate GSD installed into, then bare existence, then probe[0]. Behavior is byte-identical when probeMarker is absent (windsurf etc. unaffected). Adds detectAntigravityDirAmbiguity() for installer/operator guidance on already-misinstalled users (auto-relocation ruled out per ADR-0008's single-configDir migration bound). Regression tests fail before / pass after: coexistence + marker-priority cases, end-to-end through the registry descriptor. Design rationale: - REGISTRY (one entry: agent-skills → src/init.cts → tests/agent-skills.test.cjs) is the canonical registration site; new verbs are added here. - For each registered verb, the guard asserts its test file contains BOTH a `configured_empty` assertion AND a `not_configured` assertion — proving the configured-empty-vs-not-configured contract is explicitly tested. - NOT a universal static detector (intractable / false positives) — mirrors the no-adhoc-markdown-parsing grandfather pattern. - Uses scripts/lib/allowlist-ratchet.cjs (assertWithinAllowlist) so stale allowlist entries fail (ratchet-down) and novel offenders always fail. - checkRegistry() is factored as a pure exported function tested in tests/lint-resolution-provenance.test.cjs without shelling out. - Wired into lint:ci (package.json) and lint step name updated in test.yml. - CONTEXT.md ### Resolution Convention extended with P4 guard sentence. - Allowlist starts empty ([]) — agent-skills already has its tests. Narrows P3 of ADR-1411 (Resolution Provenance, epic #1411) based on an adversarial fit-analysis that showed a single Resolution<T> envelope adopted by agent-skills, capability-state, and capability-writer fails the deletion test: configured/reason are meaningless for capability verbs, and capability-writer's errors[] (operation-not-applied) cannot fold into warnings[]. The only genuinely shared seam is warnings: string[]. Changes: - src/resolution.cts: new pure types+builder leaf — exports Resolution<T> {value, configured, reason, warnings}, makeResolution<T>() builder, and AgentSkillsValue {block, skills_count}. No other src/ imports. - src/init.cts: cmdAgentSkills --json IR gains additive value:{block, skills_count} field (built via makeResolution). All existing flat fields (agent_type, block, skills_count, warnings, configured, reason, source, degraded) are retained unchanged for back-compat. - src/capability-state.cts: doc comment on ResolveCapabilityRuntimeStateResult naming it the canonical read-verb envelope. Add heuristic (4) to findProjectRoot: after heuristics (1)-(3) (sub_repos, multiRepo, .git+parent-.planning/) are exhausted without a match, perform a second bounded walk-up within FIND_PROJECT_ROOT_MAX_DEPTH to locate the nearest ancestor directory containing a .planning/ subdirectory. Returns that ancestor as the project root, so loadConfig finds the correct config instead of falling through to defaults when gsd-tools is invoked from a plain descendant subdirectory of a single-repo project (#1366 cwd-drift gap). Ordering is load-bearing: the new walk runs AFTER the existing loop so sub_repos workspaces (where a child sub-repo may have its own .planning/) still resolve correctly to the parent workspace. The existing own-.planning/ guard (heuristic 0, #1362) and the depth bound (FIND_PROJECT_ROOT_MAX_DEPTH=10) are both preserved unchanged. refactor(#1308): consolidate config-key precedence engine into single owner (#1322)
Make src/capability-activation.cts the sole owner of the four-level config-key
precedence walk via a new raw-value primitive resolveConfigKey(dotKey,{config,
cwd,registry}); the boolean wrapper _resolveActivationValue and loop-resolver's
resolveConfigValues are both rebuilt on it. loop-resolver deletes its
byte-identical copy and inline re-walk and imports the engine.
resolveCapabilityRuntimeState no longer returns registry/config (leaked internal
detail); the caller loads one fail-closed config snapshot and threads it in via a
new optional configOverride param, so capability `active` and hook when/configValues
resolve against the same object. capability-writer requires the registry module
directly.
Adds a DEFECT.GENERATIVE-FIX parity gate (identity + behavioral matrix +
end-to-end resolveLoopHooks configValues) that fails if the two precedence
surfaces ever diverge. Pure internal refactor, no user-facing change.

Part B: loop-resolver hook rendering now gates on capability state.active (=== true, fail-closed) instead of state.enabled, so the capability config gate is honored by the hook consumer, not just per-hook 'when'. active is now required in the loop-resolver input types. Regression test proves a config- disabled (active=false) capability's unconditional hook is not rendered. Part of #1302. Fixes a latent claude-hardcoding in the resolver: resolveCapabilityRuntimeState now detects the active runtime via resolveRuntime(cwd) (GSD_RUNTIME -> config.runtime -> 'claude') so non-Claude runtimes (Codex/Cursor) read their own surface, not ~/.claude. Hermetic regression test proves config-on+unsurfaced -> disabled; cross-runtime test proves GSD_RUNTIME=codex honors CODEX_HOME. Gate fails closed on every error path. Part of #1302.
